AN UNINTENTIONAL ROOF FALL OCCURRED ON E-MAINS, #5 ENTRY IN BY SPAD 6494. IT MEASURED 12' HIGH, 18' WIDE AND 60' LONG
A roof fall occurred in the #2A return entries at 29 xcut. The fall is between #7 to #8 entry. The fall is approximately 70'L x 18'W x 10'H. The fall will not be cleaned up.
Elevator was shut down to adjust switches. Elevator was put back into service at 7:15 p.m.
ON DECEMBER 30, 2004 THE JUSTICE NO. 1 ELEVATOR WAS DOWN DUE TO A GROUND IN THE SYSTEM, APPROX 102 MIN.
A ROOF FALL WAS DISCOVERED MEASURING 20'W, 20'L, & 8-10 FT H, BEGINNING AT THE INTERSECTION 1 BREAK OUTBY SPAD NO.1027. NO EQUIPMENT WAS INVOLVED, NO VENTILATION WAS DISTURBED, NO INJURY OCCURRED & THE FALL WILL NOT BE CLEANED UP.
A roof fall occurred in the third west parallel return at xcut 52 between #4 & #5 entries. The fall is approximately 20'L x 18'W x 8'H. The fall will not be cleaned up.
A conveyor belt fire was discovered and extinguished on the morning of 12/29/04. Due to problems with water and freezing conditions, it took approximately 1 to 1 1/2 hours to extinguish. Exact cause is being investigated.
Their was not any injury(s) or equipment damaged nor did it impede ventilation. The roof broke above the bolting horizion. Location 34 North @ 4 East. Size of the fall is approximately 60ft X 80 ft X 16ft.
AN UNINTENTIONAL ROOF FALL OCCURRED ON A-MAINS #1 TRACK NEAR SPAD 37, #3 ENTRY. IT MEASURED 6' HIGH, 8' LONG AND 18' WIDE.
Roof fall occurred in the 8th Submain East entries on the supply road in the pillar line between crosscuts #23 and #24. The area was mined approximately 10+ years ago and was supported with 8' tension rebar roof bolts. Plans are to cleanup the fall and install arches.

ROOF FALL, 3RD PANEL WEST (#5 UNIT RETURN) AT SPAD 64 AND 80 IN #1, #2 AND #3 ENTRIES. APPROX 160' WIDE, 18' LOND, 5-6' THICK
A roof fall occurred on the 2L4W section, on the 3C panel, at cross cut 11, entry #5. The cause of the fall was weak laminated sandy shale commonly referred to as stack rock.
Guide came loose in shaft causing skip to come off the guides damaging two sections in the shaft. Mine evacuated. Down Time 9:12 p.m., 12-28-04 to 8:00 a.m., 12-29-04.
At 8:59 am there was an ingnition of coal dust within the #2 coal mill ductwork system. There were no personal injuries or equipment damage. The safety rupture disc worked as designed and relieved pressure from the system created by the explosion. Foreign metal found in the coal mill is the suspected ignition source.
A roof fall occurred in the unit #1 B return entry #3 at # 5 xcut. The fall is approximately 20'L x 20'W x 10'H. The area was supported with 10' bolts. The fall will not be cleaned up.
AN UNINTENTIONAL ROOF FALL OCCURRED ON A-MAINS, #1 TRACK AT #8 BLOCK. IT MEASURED 7' HIGH, 18' WIDE AND 20' LONG.
A FALL OF GROUND OF APPROX. 30 TONS OCCURRED AT THE INTERSECTION OF C52-D4 & THE CENTRAL ORE DRIFT DURING A SHIFT WHEN NO PERSONNEL WERE WORKING UNDERGROUND. FALL WAS IMMEDIATELY ADJACENT TO AN AREA THE CREW HAD BEEN INSTALLING GROUND SUPPORT IN BUT DID NOT INVOLVE THE AREA WHICH HAD ALREADY BEEN SUPPORTED. NO EQUIP. DAMAGE OCCURRED. MSHA WAS NOTIFIED BY PHONE.
A rock fall occured in the #6 entry 4 breaks inby #7 head-drive. The fall measured 19'W X 60'L X 4'H.
A MINE FIRE WAS DISCOVERED AT OR NEAR THE CONVEYOR BELT SLOPE ENTRY NEAR THE BOTTOM OF THE SLOPE. NO ONE WAS UNDERGROUND WHEN SMOKE WAS DISCOVERED COMING FROM THE MINE. FIREFIGHTING ACTIVITIES HAVE BEGUN AND ARE CONTINUING. THE TIME OF THE ACCIDENT IS THE APPROX. TIME SMOKE WAS DISCOVERED. NO ONE WAS SCHEDULED TO WORK ON THIS SHIFT.
Piece of metal broke loose where ore skip dumps in and fell into shaft. Damaged guides were replaced and shaft back into service at 6:15 a.m. on 12-25-04. Down Time 2:30 p.m. 12-24-04 to 6:15 a.m. 12-25-04.
HIGH WALL ROCK FELL DUE TO POSSIBLE WEATHER, RAIN AND FREEZE TEMPERATURE CHANGES.
A ROOF FALL HAS OCCURRED IN THE #1 ENTRY OF OUR WEST MAIN SECTION BETWEEN SPAD 87+75 AND 86+65. THE FALL MEASURES 20' WIDE X 30' LONG X 8' HIGH. THE FALL IS LOCATED BETWEEN CROSSCUT 81 AND 82 APPROX. 700 FT. OUTBY WEST #1 SECTION.
Loss Electrical power to underground mine due to ice/snow storm. Power came on and off several times until around Friday morning, 12-24-04, it finally remained on. The mine was evacuated during that time until electrical power was restored. No one was hurt or injuried.
An unintentional roof fall occured in the intersection between no.7 & no.8 entries in the return airway 800' outby the section located at spad no.2613. The fall measured 25'L x 25'W x 6-7' thick. The airway or travel is not affected by this fall.
An unintentional fall of roof occurred on the Elk Creek Mains two breaks inby spad #395. The fall was in a crosscut between #4 and #5 entry and was app. 20' x 20' x 4'.
Roof fall occur in crosscut between No. 3 and No. 4 entries near spad No. 1802. The rock is 35' long, 20' wide and 6' to 8' thick. The area is near the outcrop with low cover, water is present in roof strata along with roof cracks.
The garden b-shaft automatic elevator was inoperable due to a malfunction in the control circuit.

ROCK FALL OCCURRED IN #5 ROOM 1+20 FT. OUTBY WORKING FACE, 300 FT. INBY DUMPING POINT. NO WATER OR ADVERSE CONDITIONS COULD BE SEEN.
An unintentional roof fall occurred on a-mains, 39-40 block, (#2 track) atspad 332. It measured 6' high x 18' wide x 20' long.
An unintentional roof fall occurred on e-mains, #5 entry, 120' inby spad 7833. it measured 8' high x 18' wide x 25 long.
A ROOF FALL OCCURED ON #2 UNIT(2ND SW SUB-MAIN) AT X-CUT #8 BETWEEN #5 AND #6 ENTRIES. THE FALL MEASURED 18' WIDE, 85' LONG AND 6' HIGH.
An unintentional roof fall occurred at the Galatia North Portal, at the #2 Entry of the 13 West Headgate, 3140' West. The fall was approximately 30' long, approximately 25' wide and approximately 18' thick. The fall did not affect ventilation or passage of workers. The fall will be cleaned up.
THE ELEVATOR AT THE #6 PORTAL WAS INOPERATIVE FOR 55 MINUTES DUE TO A FAULTY DOOR SWITCH.
A rockfall occured in the Main North at crosscut #9 in the intersection of #6 entry. The fall measured 20'X20'X6'. This area was mined in September of 1996 and was supported with 6' tension rebar bolts. The fall was caused by an undetected slip.
Roof fall on No. 3 belt in neutral entry across from spad # 111. Fall is estimated 50 feet long, 20 feet wide and 5 feet high. Set Hientzman jacks on all sides.
The elevator at our Hart street portal failed to operate properly. After inspection and cleaning procedures the brakes on the elevator would not hold the cage in one position at the top and bottom landings. It was determined that carbon blown off the motor armature was deposited on the brake pads of the elevator causing them to slip.
A roof fall was found at 12:30 am on 12/20/04. located in a four way intersection of entry #0 corss-cut 173, Main south. Fall was 20'w x 20'l x 10' H, area was mined Jan. 1997 and had been supported with 8' full resin bolts. no injury or property damage resulted. no current plans to clean up the fall.
The Alimak elevator at the East Hornsby Air Shaft became inoperable when the door switch failed at 10:45 AM on 12/19/2004. Investigation revealed that the door switch was out of adjustment due the cold weather. The door switch was adjusted and the elevator was placed back into service at 11:50 AM.
An unintentional fall of roof occurred on #9 belt at spad #2275. The fall was approximately 20' x 40' x 4'-6' thick.
A pulley on the bottom of the elevator failed resulting in damage to the counterbalance chain. Both the pulley and counterbalance chain have been replaced.
An unintentional roof fall occurred in e-mains in #5 entry track, 2 1/2 blocks inby spad 8110. it measured 5' high x 20' long x 18' wide.
An unintentional roof fall occurred, on l-Mains, MMU-007, in #2 entry, just inby spad 10964. it measured 8' high x 18' wide x 100 long.
A ROOF FALL WAS DISCOVERED 600' OUTBY THE NORTH MAIN SECTION, MMU# 009 & MMU# 008. AREA AFFECTED MEASURED 30'L X 90'W X 4.5-5'THICK. THE AREA HAD BEEN SUPPORTED WITH 4,5 & 6'TENSION BOLTS.
Encoder shaft slipped on drive coupler, resulting a rope speed fault. When HSS control power was reset to clear the fault, the encoder entered a faulty value for the lower limit overtravel, making the hoist inoperable until the corrected value could be entered into the computer programming system. Due to travel time, the hoist was inoperable for more than 30 minutes.
BASED ON THE INFORMATION RECEIVED FROM THOSE PRESENT IT APPEARS THE ALLEDGED IGNITION OCCURRED WHEN THE SPARKS FROM THE CUTTING MACHINE (SHEAR) IGNITED AN UNKNOWN METHANE BLEEDER WHILE CUTTING HARD ROCK AND COAL IN ADVERSE GEOLOGICAL CONDITION.
ELEVATOR OUTAGE OCCURRED AT EAST FINLEY PORTAL WHICH LASTED MORE THAN 30 MINUTES. THERE WAS NO ENTRAPMENT OF PERSONS. A HORIZONTAL BAR WHICH OPERATED THE SAFETY DOGS BECAME BENT AND WAS REPLACED.

Grease and coal dust around a bearing located on a McLanahan crusher caught fire which ignited suspended coal dust. This resulted in hot embers being drawn into the dust collector system causing an ignition of coal dust and a fire in a bag house.
The coal mill system shut down & on restart we experienced a coal mill explosion. The explosion was attributed to smoldering coal under the bowl of the coal mill.
An unintentional roof fall occurred at the Millennium Portal, at the 4N Longwall Tailgate (3 North Headgate Entry #1 @ 914 North, Crosscut 18 (Blocked tailgate)). The fall's length, width and thickness are unknown. The fall did not affect ventilation. The fall will not be cleaned up.
A fall of supported roof was found during weekly exam in an abandoned area of the mine. The fall was located at #66 break on south mains #5 & #6 entry. The fall measured 20' x 90' x unknown height.
A roof fall occurred in #1 entry of the 2nd mine at survey station 41+50. The fall measured 110' long x 18' wide x 9' thick. The area had been supported with 6' grouted and 9' double lock bolts. All approaches to this area have been timbered off.
An unintentional roof fall occurred on MMU-007, #4 entry, 1 block inby spad 11280. It measured 6' high x 18' wide x 16' long. It did not affect an airway and was not in an intersection.
Railroad Commission Inspector was driving out in the tall grass at the corner of 3B haulroad and main haulroad. The exhaust on his truck started the grass on fire. We had a water truck to wet the roadside and powerline pols to prevent fire from jumping across the haulroad. It took approximately 45 minutes to extinguish the fire.
An unintentional roof fall occurred at the Millennium Portal, at the 4N Longwall Headgate Entry #2 at 2352 North, Crosscut #33. The fall was approximately 50' long, 30' wide and 8' thick. The fall did not affect ventilation. The fall will not be cleaned up.

A fall of supported roof was found during weekly exam in an abandoned area of the mine. The fall was located on No. 8 panel #3 crosscut off south mains. The fall measured 20'W x 300'L x unknown height.
An ignition occurred on the 2-east off of 3-north mains section at the face of #4 entry. the ignition lasted approx. 15 seconds and contained to the face area. the ignition was extinguished with wash down hose. there were no injurieds and there was no. damage to equipment.
Production hoist rope slip-tach failed during production hoisting. Repairs made to slip-tach coupling. This is located on the deflection wheel on surface. Operational at 2:45pm.
PULLING PILLARS IN THE #5 ENTRY. MINING THE PUSH OUT WHEN THE ACCIDENT HAPPENED. THE 4 MRS JACKS WERE SET IN PLACE, NARROWING DOWN THE ENTRY TO 16 FT. LOADED THE LAST S/CAR WHEN FALL OCCURRED.
A ROOF FALL OCCURRED AT THE INTERSECTION IN FRONT OF THE #1 SEAL OF 17 EAST. THE FALL WAS 20' LONG X 18' WIDE X 8' HIGH. AREA WAS MINED IN JULY, 1995. FALL OCCURRED DUE TO THE DETERIORATION OF THE IMMEDIATE ROOF. AREA WAS SUPPORTED WITH 8' POINT ANCHOR BOLTS.
The supervisor of the miner section found a roof fall that was above the anchor point of the roof bolts in an ""ouby"" area of the mine. The fall was approx. 15' wide, 30' long and approx. 4' high. The roof bolts used in this area were 42"" mechanical that were installed 20+ years ago. The fall did NOT impede travel or ventilation.
A fall of roof occurred at Crosscut 36, Entry 5 of the 1st North off the Main East. The fall measured approx. 30-40' long x 16' wide x over 9' thick. The fall did not result in any damage to equipment or property.
A roof fall occurred at break 73 on the Big 4 belt. The fall measured 20' wide x 25' long x 8' thick.
Air flow safety device detected improper air/gas mixture and automatically shut down boiler which powers generator for power generation. Secondary power to the hoist was lost and evacuation commenced at 3:25 am and was complete at 3:39 am. Power was restored at 3:50 am, the hoist was tested and miners returned underground 4:00 am.
Fall app 71' long 25' wide 0-12' thick, caught belt drive started box, track- will be cleaned up. Fell in secondary escapeway, no blockage of ventilation, no injuries, area re-bolted with 6' torque tension and cable bolts, cribs set, plotted on mine. Clean up plan posted.
A roof fall occurred in the 1 west #3 panel, #3 entry, crosscut #1b, intersection. the fall measures approx 30 feet long, 18 ft wide, and 10 ft thick. the area is supported using 48 inch full grouted roof bolts and 10 foot point anchor bolts.
DURING THE DUMPING CYCLE A ROCK LODGED IN THE GATE MECHANISM OF #2 SKIP, CAUSING THE SKIP GATE TO LODGE AGAINST THE SHAFT DUMP ASSEMBLY. THE SKIP HAD TO BE MANUALLY EMPTIED TO FREE THE SKIP GATE. NO DAMAGE OCCURRED TO THE SKIP OR SHAFT AS A RESULT OF THE INCIDENT. MSHA NOTIFICATION WAS MADE IMMEDIATELY.
Roof fall due to mining right at 100 foot cover line close to outcrop. Knew top was getting surface cracks and some mud streaks so went ahead and moved section.
HOISTING ENGINEER WAS LOWERING ON MANUAL. DOG ON SOUTH WEST CORNER OF CAGE CAUGHT, WHEN RAISED IT DID NOT RELEASE PROPERLY. BROKE THE DOG & LINKAGE. REPLACED 2 DOGS AND LINKAGE.

A roof fall was found Dec 10, 2004 at 11:30 PM at survey station 29 and 99 entry #2, main south. Fall was in a four way intersection that had been mined June 6th 2004 and supported with 8' full resin bolts. fall measured 15' long and 15' wide and 8' high. No injury or property damage resulted. Fall will be cleaned up and resupported.

ROOF FALL WAS DISCOVERED IN THE 4TH NW SUBPANEL IN #2 ENTRY AT XCUT 4 ON #4 UNITS NEW SEP UP. THE FALL DID NOT AFFECT VENTILATION, PERSONEL OR EQUIPMENT. THE FALL MEASURED 85' X 20' X 6'.
ROOF FALL OCCURED IN THE 4TH SW SUBPANEL, IN #2 ENTRY IN THE INTAKE AT XCUT 7 ON #2 UNIT. NO PERSONEL, EQUIPMENT OR VENTILATION WAS AFFECTED. THE FALL MEASURED 69' X 19' X 6'.
A roof fall occured in the x cut 1x2 for a distance of app. 22 feet 15 feet wide and 7 feet above the coal. the area outby was bolted with 6 foot torque tension bolts. the face had not been bolted propr to the fall. it was bolted with 8 foot combination bolts and the area screened.
An unintentional roof fall occurred on MMU 007 #1 entry 1 1/2 blocks in by spad 10783. It measured 6' high by 18' wide by 70' long. additional floor to roof support was added.
An unintentional roof fall mesuring 80' long, 20' wide, and 5.5' high was found in the intake entry 30' outby spad 50541. The area was developed Aug. 04 with 5' torque tension roof bolts in place. The fall 1100' outby the 003 mmu and did not impeade travel or affect ventilation. The fall perimeter was resupported and barricaded to prevent travel into the fall.
An unintentional roof fall occurred in #2 entry, 1 block out by spad 10847, on MMU-007. It measured 10' high by 18' wide by 112' long. Additional floor to roof support was added.
An unintentional roof fall occurred on MMU- 007, #4 entry, in by spad 10902. it measured 6' high by 18' wide by 75' long. The fall area will be cleaned, rebolted and re-screened.
ON 12-08-04 A ROOF FALL OCCURRED ON 030 SECTION. THE FALL MEASURED 6' HIGH, 20' WIDE & 20' LONG. THERE WERE NO INJURIES REPORTED IN RELATION TO THIS ROOF FALL.
An excessive amount of rainfall Tuesday, December 7, 2004, caused flooding of the quarry at this location. Weather permitting, normal operation is expected to resume Wednesday afternoon, 12/08/04.
Unintentional roof fall in the #1 entry of old 1st. Right Mainline - Mouth of Old Glory. Length unknown X 19' wide X 7' High. Spad #893.
A roof fall occurred in the #3 entry of the #3 working section, 1 crosscut inby survey station #15609, approximately 800 feet outby the working face. The fall started in the intersection and measured 30 feet long, 18 feet wide, 8 feet high. The area will not be rehabilitated.
As the dayshift crew were beginning to descend down the shaft, an electrical contactor malfunctioned causing the elevator to stop one foot below the top landing platform. After being stuck for 36 minutes the 9 man crew was removed and the elevator was repaired.

An unintentional roof fall occurred on East mains, in #5 entry, in by spad 10394. It measured 8' high by 18' wide by 45' long. This entry was an escapeway, which was re-routed and additional floor to roof support was added.
AN UNPLANNED ROOF FALL OCURRED ON SOUTHWEST SECTION 15000' FROM ACTIVE FACE. AREA DEVELOPED 1993 SPAD. FALL MEASURED 60' X 16' 3'. NO ACCIDENTS REPORTED
A fall of supported roof has occurred out by the active section on #6 belt entry in by spad station #1493. The fall measured approx 20' by 60' by 10' high. No injuries resulted, however the conveyer belt was damaged.
An unplanned roof fall occurred on the #2 section #4 entry, 1 break outby spad #5009. The fall fell 2,900 ft. from the active face. The fall measured 80' x 20' x 8'. Six foot torque tension roof support was being used.
AN UNINTENTIONAL ROOF FALL OCCURRED ON 3RD EAST #6 ENTRY, #74 X-CUT. THE FALL WAS 7' HIGH X 15' WIDE X 20' LONG. FALL WAS DUE TO ADVERSE ROOF CONDITION ABOVE ANCHORAGE OF BOLTS.

Roof fall in face and intersection of #3 entry, break 2, 3-2 xcut to the left, 1-left mains development. Intersection and faces were ok at pre-shift inspection. No one on section during shift due to work needed outside. Upon returning to faces, fall found. Roof material found laying against back of bolter parked in 3-2 break 2 xcut to left.

Mine Examiner was checking returns and found rock fall which originated from the first break of the #3 entry, #5 left panel and through the #1, 2 & 3 entries of the #1 West Submains off the #1 North Main. The rock fall measured 170' x 22' x 6'.
LIVINGSTON ELEVATOR WAS OUT OF SERVICE FOR MORE THAT 30 MINUTES DUE TO MALFUNCTION ON OVERLOAD SWITCHES. ELEVATOR WAS DOWN WITH AN EMPLOYEE ON IT - FROM 3:40 PM TO 4:25 PM. SCHINDLER ELEVATOR SERVICE DID NECESSARY ADJUSTMENTS ON ELEVATOR AND INSPECTED IT THEN RETURNED IT TO SERVICE.

Located in Hatfield Mains in return #3 entry at x-cut 22. Continuation of an old rooffall reported in 6-4-02. Area was already cribbed and dangered off to impead travel. Fell on downhill around 50'x60'x8' hi. Area was bolted with 6' fully grouted resin bolts.

FORWARD & REVERSE RELAY SWITCH MALFUNCTIONED ON SLOPE HOIST CAR.
EE was fatally injured when struck by ice dislodged from a slurry pipeline.
EE was changing tire on 10SC shuttle car, had new tire on and was finishing tightening bolts, when the tire exploded and the 2 safety rings struck the victim in the face, left hand and chest area.
The individual was discovered in a structure designed to settle aggregate fines. His job duties involved keeping the area clean from spillage & debris. The Structure wall is over 4 feet high and there was no reason for the individual to have access to the top of the structure. He was discovered floating in the structure.

ONYX USED HIGH PRESSURE WATER TO CUT INTO SCALE. ONCE TECH STARTED, SCALE GAVE WAY & TECH WAS TRAPPED UNDER LIQUIDE THAT CAME OUT. MORE LIQUID WAS IN TANK THAN THOUGHT. CO DID NOT SOUND TANK.

Treecutter was cutting a tree that had another tree leaning into it. Cutter was fatally injured when the leaning tree fell on him.
Employee on 12/29 received treatment for an injury he reported on 11-10-04. At the time he was assisting in the replacement of conveyer belt skid plates. While pulling a skid plate out of position for replacement he felt a pull in his groin area.
WHILE THE EE WAS OPENING A DOOR TO A PRESSURIZED ROOM THE DOOR SLIPPED FROM HIS HAND CAUSING IT TO PINCH THE TOP OF HIS LEFT MIDDLE FINGER
Employee climbed up escapeway tubing in the glory hole about 100' to work on a gob switch. As he was descending through a 2'x2' door, the outside man started the #2 surface belt. The coal falling into the chute made air pressure slam the door shut on his right middle and ring finger. The tips of the fingers had to be amputated.
EE PROCEEDED TO INSPECT THE CLAY SHREDDER. HE TURNED THE SHREDDER OFF BUT DID NOT LOCK IT OUT. THE EE DECIDED TO CLIMB INTO THE SHREDDER TO INSPECT FURTHER. ANOTHER OPERATOR ON THE SHIFT WALKED BY & NOTICED THE SHREDDER WAS OFF & PRESSED THE START BUTTON. THE EE FAILED TO LOCK OUT THE SHREDDER BEFORE PERFORMING WORK ON IT.
EE WAS ATTEMPTING TO CLEAN AIRLOCK WITH DISCONNECT ENGAGED AND NOT LOCKED OUT. AMPUTATED MIDDLE FINGER AND INDEX FINGER ON RIGHT HAND - BOTH AT CENTER OF MIDDLE PHALANGE.
the operator was in the back and ee in front attaching the pull-rope operator stopped at curtain, shut machine off, & checked for person on other side.he then turned machine on & raised pan, not knowing the ee was attaching rope to the stop. the ee's little and ring finger was caught between the pan & stop. fracture, laceration to both fingers amputation to tip of little finger.
EE said that he was holding metal door open while a coal hauler was passing through, the coal hauler bumped the door, the door sprang and caught fingers against fan house.
While EE & partner were lowering & setting an air slusher in a drift the slusher became unbalanced about 6"" off the ground. EE put his hands on slusher to push it over before his partner began to lower it, when he lowered it the cable slipped & moved over to where his L hand was. This created a pinch point & resulted in EE getting his little finger crushed & ring finger fractured.
REPLACING MOTOR ON HEATER PUSHING ON MOTOR WITH TWO FOOT PRY BAR AND TRYING TO TIGHTEN BOLT WITH OTHER HAND, FELT BURNING IN LEFT SIDE OF ABDOMEN. RCVD DIAGNOSIS 12/20.04, NOT REPAIRED AT THIS TIME
EE was dragging water hose when they stepped on the miner cable, that had some oil and water on it, slipped and fell face down. Landed on both elbows, his hip, and knees causing them to hyper-extend in the abdomen area. They saw a Physicians Assistant who referred them to a surgeon that diagnosed them with a hernia. They hasn't lost any time (12-23-04) for hernia.
THE INJURY OCCURRED WHEN TWO MAINTENANCE EMPLOYEES WERE USING A TWO WHEELED DOLLY TO MOVE A MOTOR GEAR BOX ASSEMBLY FROM THE MILL LEAD FILTER TO THE MAINTENANCE FACILITY. THE EMPLOYEE WHO SUFFERED THE INJURY WAS ATTEMPTING TO PULL THE DOLLY AROUND A CORNER WHEN THE LOAD SHIFTED ON THE DOLLY PINCHING HIS FINGER BETWEEN THE GEAR BOX AND THE DOLLY FRAME.
MECHANIC WAS STANDING BETWEEN SCOOP BUCKET AND MINER RIPPER HEAD. SCOOP DRIFTED 2 FEET AND SMASHED HIS LEG. #7 ENTRY L.O.C. 2 NE E PANEL.
Employee was working on valve, caught his finger while valve was being closed.
Employee was positioned out by the continuous miner (x-cut out by block being mined) when a piece of draw rock fell between roof bolts and rib striking employee.
EE injured left leg and ankle when left rib rolled while operating Roof Bolter. He worked his next 4 scheduled days and missed work on 1/12/05, his first scheduled day not worked.
EE WAS HELPING RECLAIM HV CABLE ON #2 UNIT WHEN FOOT GOT UNDER THE SCOOP BUCKET AS IT TRAMMED FORWARD CAUSING A FRACTURE TO RIGHT FOOT.
Employee injured his left eye while he was letting pressure off of a grease jack on the #3 mobile bridge conveyor. The grease fitting blew out and struck him in his left eye.
Employee stated his left knee caught belt rail. He told operator of utility loader to lower bucket; instead, the operator raised the bucket and the employee sustained a fracture.
EE FELT PAIN IN HIS LOWER BACK AS HE BENT OVER & THREW CABLE OUT OF ROADWAY
Employee was drilling a roofbolt hole on the right side of the miner when a piece of coal broke out of the top striking him on the left side of his head and face. The top hit his glasses causing a laceration to his left cheek.
EE SAID HE FELL TO HIS KNEES CAUSING BRUISING.
EE BENT DOWN TO PICK UP A CRIB BLOCK AND PULLED A MUSCLE IN LOWER BACK
WHILE EE WAS INSTALLING BELT STRUCTURE EE EXPERIENCED PAIN IN RIGHT SHOULDER. EE CON'T WORKING UNTIL 3/1/05 AT WHICH TIME HE WENT OFF TO HAVE SURGERY PERFORMED
EMPLOYEE WAS LOADING B.O. PARTS AT THE SHOP WHEN HE FELT PAIN IN HIS LOWER BACK AND GROIN.
EMPLOYEE WAS GOING TO WORK ON HAWKEYE AT THE SECTION TAILPIECE. HE SHUT BELT OFF AT TAILPIECE. BEFORE THE TAILPIECE QUIT ROTATING, HE REACHED IN TO PERFORM WORK. HE CAME INTO CONTACT WITH THE TAILPIECE WITH HIS LEFT MIDDLE FINGER. THE LEFT MIDDLE FINGER SUSTAINED A FRACTURE.
DRIVING AN EMPTY FUEL TRUCK, TRUCK POWERED OUT ON HILL. HE TRIED TO RESTART THE TRUCK AND IT STARTED GOING BACKWARDS DOWN HILL. TRUCK HIT BANK AND ROLLED OVER. CUT FOREHEAD AND FRACTURED SHOULDER BLADES.
EE WAS DRIVING A PIN INTO A SHOVEL TRACK USING A LARGE SLEDGE HAMMER AND STRAINED HIS BACK.
Employee was operating a shuttle car on the 003 pillar section. As he returned to the miner he ran over a piece of rock causing him to bounce up and hit his head on the inside of the canopy injuring his neck.
EE WAS WALKING AROUND AND ROCK FELL FROM THE ROOF AND KNOCKED HIM DOWN.
Employee was roof bolting when he caught his left thumb between the steel, breaking and bruising the skin along the left side of the thumb.
EMPLOYEE WAS USING HIS FOOT TO GUIDE THE BELT SHEAVE ROPE ONTO A BOTTOM ROLLER WHEN HIS FOOT BECAME ENTANGLED IN THE ROPE. THIS CAUSED A FRACTURE OF THE LOWER LEG. THIS RESULTED IN LOST WORK DAYS.
Employee was operating a 992G Loader, a rock fell from the highwall hitting the loader on the boom causing a severe jolt. EE felt pain in his neck & shoulder.
EMPLOYEE WAS LIFTING A WET BRAKE OUT OF THE RAIL RUNNER WHEN HE CUT HIS RIGHT HAND FROM A PIECE OF METAL ON THE BRAKE. HE WAS TRANSPORTED TO THE HOSPITAL AND RECEIVED 4 STITCHES TO HIS HAND.
12/29/04 EE MADE A SECTION BELT MOVE AFTER EXITING THE MINE HE COMPLAINED OF SORENESS IN HIS LOWER BACK. HE WORKED ON 12/30/04 & TOLD HIS SUPERINTENDENT THAT HE WOULD BE OFF DUE TO OTHER COMPLICATIONS. 0N 1/10/05 DUE TO SORENESS EE VISITED A DR & WAS TOLD THAT HE MIGHT HAVE A PINCHED NERVE.
Employee stated he was sliding a continuous miner cat idler out of the back of a supply truck onto the forkds of the warehouse forklift when the idler sled unexpectedly and pinched the index finger of his right hand between the forks of the forklift and the cat idler.
EE was bolting on the right side of the bolter. As he raised the bolter boom to couple everything together, this caused the drill steel to bow and when it slipped off the lip the steel flexed striking him in the right hand palm as he was reaching for the drill steel. He was not able to work his next shift because of fractured bone next to his thumb.
Employee was in the process of loading coal and stepped off the loading platform when he slipped and fell. Resulted in sprain/strain to the neck and upper back.
Two employees were in the process of changing out a two-stage hydraulic pump on a Fletcher Roof Bolter. The injured employee was working on the old pump in the rear walkway of the bolter. The other mechanic was working on the new pump on the top of the bolter (bolting tray). The pump fell from the tray and landed on the victims hand.
EE was welding on the end cap of a dozer push arm. He thought he had flashed his eyes, but it continued to get worse. He went to the doctor on Jan 7, 05, The Dr cleaned his eye and told him to stay off work on the 7th and he could return to work on the 8th of Jan. EE missed one day.
EMPLOYEE SUFFERS FROM ARTHRITIS IN HIS KNEES. UPON ENTERING AND EXITING THE VACUUM TRUCK HE TWISTED HIS KNEE, RESULTING IN INFLAMATION. RESTRICTED DUTY BEGAN 1/24/05.
Truck driver & mechanic were attempting to set engine in D11 Dozer with boom truck. Outrigger slipped off block, engine shifted pinning both legs between dozer & engine.
Individual was on knees installing slides in the continuous haulage rapid haul (lowlow) when he twisted his body in an awkard position straining his back
USING A CHAIN TO HOIST ROLLER ON TO THE FRAME ON A BULL DOZER, THE CHAIN BROKE STRIKING HIM ON THE HAND.
Employee was shoveling on the belt, crossed under the belt to set a pump and became stuck in the mud, cutting his arm and leg on piece of steel under the mud.
While using a prybar to loosen bolts, the prybar kicked back and hit employee on his leg.
While cleaning ice off conveyer EE slipped and fell 9 ft to ground.
There was a dust spill in the Amund Conveyor area. EE was sent over with the vaccum truck and he stepped into the hot dust getting burned.
EE WAS OPERATING A SHUTTLE CAR WHEN HE RAN OVER A PIECE OF COAL THAT BROKE & A PIECE WENT INTO HIS LEFT EYE. EE WAS TREATED AT THE E.R. WHERE IS WAS DETERMINED THAT HIS EYE WAS SCRATCHED.
INJURED WAS SAWING POST WHEN BUNDLE OF POST BEHIND HIM BROKE LOOSE & HIT HIM ON LEG & BACK CAUSIG A RIGHT KNEE SPRAIN & BRUISED BACK.

Employee was getting cable out from under a sled in the bucket of a scoop and the scoop driver dropped the bucket, catching the employee's top of his left foot between the sled and the bottom.

Employee was preparing to start an end dump truck that had been repaired. As he walked in front of the truck he slipped on the ice/snow and fell on his back. (diagnosis - herniated disc) This is a LTA.
Employee was putting tire chains on the service truck. He was using a block to raise the back tires. The block started to slip and the employee put his foot on the block to stop it from slipping, when the block pushed back hitting his right ankle.
The plant foreman directed ee to remove did old rollers and frames from an old conveyor to a rock conveyor they were rebuilding. he made improve decision by nut asking for help when he discovered that the old 36"" troughing roller and frame were heavier than normal.
MINER WAS INSTALLING BOLTS IN CLA WITH 3/"" DRIVE RATCHET & PULLED STOMACH MUSCLE. WAITED TO GO TO DR ON 1/20/05 DR SAID RUPTURE AND WOULD NEED SURGERY. BUT HE IS STILL WORKING TILL THEY SCHEDULE HIM A DATE
Employee was tamping a place to shoot explosives when he pulled the tamp stick backa nd caught his right hand on a roof bolt.
OPERATOR OF D11 DOZER(CO#D12) WAS CLEANING WINDOWS DURING LUNCH BREAK. EE STEPPED BACK AND STEPPED OFF THE CATWALK FALLING TO THE TRACK THEN FALLING AND HITTING PUSH ARM THEN TO THE GROUND.
Employee was working an un-plugging the hammermill and was int he process of opening the cover. the cover pivole up on hinges. the cever went over center and struck the employee on the right thight causing extersive and severe bruising.
Employee was repairing 988F that would not start due to the cole; in effort to start machine, had to use the hand priming pump and while pumping strained right shoulder.
Employee was getting out of a pick-up, took one step and didn't land on foot level. The left foot bent over and fractured ankle.
While refueling the level of fuel spilled out over the tank. Climbed down and stepped across to the top of tool box and lost footing, cause of fuel on bottom of boots and fell to ground landing on feet.
Employee was walking down a snow covered area towards pit mouth to put scoop on charge when he fell on a piece of sheet metal that was covered with snow, injuring his left hip.
Employee was helping in moving a 55 gallon oil drum from a wood pallet located at the warehouse entrance; next to the pallet was a garbage metal container. When the employee rotated the drum it turned over and caught his right-hand thumb against the metal container, crushing his thumb.
The employee was hooking slings from an overhead crane to a group of granite slabs. The slabs shifted causing the employee to step backwards into a drain ditch, resulting in a twisted right knee.
DESCENDING FROM A STEP STOOL. EE MISSED THE SECOND STEP, FELL BACKWARD, AND HIT HER HEAD ON THE EDGE OF THE DESK. EE'S IMPACT ON THE EDGE OF THE DESK CAUSED A 3/4"" LACERATION TO THE BACK OF THE HEAD.
A large rock dropped into bed of haul truck shaking the truck hard enough to hurt employee's neck.
EMPLOYEE ALLEGES HE WAS LAYING OUT HOLES AND SLIPPED ON THE ICE.
Employee was helping another employee wash parts, a spacer off of a 992C Cat Loader was on the bench beside the parts washer. The spacer fell off or was knocked off and hit the floor and then bounced up and struck the employee on the inside of his left ankle.
Pulling belt slack with an air tugger, the tugger slipped and struck him on the left side of the face and head causing laceration and contusion.
The employee was taking a load of blue Aqua-line into the 12th East section. The load made contact with the pipe that was already in place and the pipe that was already hung, broke free of the tie wire securing it and hit the employee in the head.
Employee reached down to move his tool tote tray and felt a popping sensation in his lower back. Medical treatment was sought on 12/26/04 and he was taken off work.
EE was backing bridge system out of a cut, while doing so the outby edge of operator's canopy caught a roof bolt head, throwing EE against arm rest and panic bar, causing injury.

Rick was walking down the belt entry and felt an achy pain in his right foot. He was rockdusting and patrolling the belts.
Employee stepped out of crane onto a pile of rocks causing him to slip, twisting his left knee. Taken off work starting 1/19/05 pending surgery to repair miniscal tear.
The employee was working with an air operated heavy impact wrench to tighten a nut. The tool came off while rotating and struck employee's right hand. Injury is now resulting in Lost Time effective 5/9/2005 due to surgery on that date.

HE EXPERIENCE A TWINGE IN LOWER LEFT ABDOMEN. IT WAS LATER DIAGNOSED AS A HERNIA 3/15/05 STARTED MISSING SCHEDULED WORK DAYS ON 3/18/05 BECAME REPORTABLE ON THIS DATE
Employee claims that while operating a coal hauler she hit a hole in the ramp which caused her to bounce in the seat. Employee sought medical treatment on 12/23/04.

WHILE REMOVING HOIST CABLES FROM THE EQUALIZER, THE SHOVEL CABLES CAME LOOSE AND FELL, STRIKING EE ON THE RIGHT HAND. FIFTH FINGER (PINKY).

EE WAS GETTING OFF THE FORKLIFT ON THE SURFACE AND SLIPPED AND TWISTED HIS LEFT KNEE. HE WORKED FROM 12/21/04 TO 1/21/05 BEFORE HE WENT OFF ON HIS KNEE INJURY.
WHILE SEALING THE HIGHWALL WITH THE HOE BUCKET, THE INJURED'S HOE SLIPPED ON THE WALL, JERKING THE OPERATOR & STRAINING HIS BACK.
Injured person was walking downhill on a sloped roadway when he slipped on fell. he instinctly held out his right arm to cushion his fall which jammed his right shoulder when his arm hit the ground. He sustained an injury to the right rotator cuff which is going to require medical treatment.
EE WAS OPERATING THE DBT ROOF BOLTING MACHINE IN JL CROSS-CUT. EE WAS DOWN ON KNEES OPERATING THE BOLTING MACHINE AND DROPPED THE POT ON THE BOOM DOWN ON RIGHT KNEECAP.
EE was walking at Cement Mills roof area. Suddenly he felt pain in his right knee. He continued with the pain and was sent to the State Insurance Fund.
EE WAS PASSING BY SECTION SCOOP IN #4 ENTRY TOWARDS THE POWER CTR. THE RUBBER 2 MAN PERSONNEL CARRIER STRUCK THE OPERATORS COMPARTMENT OF SCOOP,CAUSING WHEEL TO SPIN STRIKING EE'S RIGHT HAND. THE EE CONT'D TO WORK WITH NO MED TREATMENT UNTIL 1/3/05 & THEN STARTED MISSING WORK DAYS. THE INCIDENT IS UNDER INVESTIGATION TO DETERMINE IF AN INJ EXIST TO PREVENT EE FROM WORKING.
Employee stated that he was trying to snag the electrical cable of a pump. When he did snag the cable he went to pull and felt a twinge on his lower back. **COMPANY WAS NOT NOTIFIED UNTIL 03-31-2005 OF CLAIM
HE WAS STRIPPING MATERIAL FROM HIGHWALL AND IT HAD CONTAINED A POCKET THAT WAS NOT VISIBLE. THE MATERIAL SLID OFF AND UPSET THE TRACK-HOE AND SHOVED IT ABOUT 100 FT. VICTIM WAS WEARING HIS SEAT BELT AND WAS TRAPPED FOR ABOUT 2 HOURS AND 25 MINUTES.
Slipped on black ice when inspecting work site. Fractured right fibula.
Bending over to lift wheel chocks. Experienced pain in lower right back. He was not yet lifting - just bent over.
A piece of rock had the cable to the roof bolting machine fouled under the deck of the machine. While trying to break the rock, the injured was holding a hammer against the rock while another person was using a hammer to strike a blow. A piece of metal flew off the hammer and imbedded into the injured's right leg.
EMPLOYEE WAS THROWING MINER CABLE TOWARD RIB AND SPRAINED LEFT SHOULDER.
The employee was operating a front end loader and as he descended the ladder, he encountered a sharp pain in his right knee as he stepped to the ground. Medical attention on 12/27/04.
Employee stated that while working in the 6 north left return-inby t.g. #2 at approx. 115 block-dragging rock dust hose his feet became entangled causing him to fall landing on his left knee-continued to work rest of his shift began missing work on 12-15-04. contusion/aprain left knee.
Carpal Tunnel Syndrome - Left Wrist This condition was reported to the employer on 1/20/05.
EE was tramming the miner across the section. He had stopped to hook a pull rope on boom of miner, when he accidently pinned himself between boom of miner & rib.
EE was moving the continuous miner from cut A to cut B in a mining cycle. He had stopped the machine to move the waterline. A piece of the rib fell and struck him on the back. The striking force of the rib pushed him into the miner tail face first causing lacerations to his forehead to his forehead and right forearm and fracturing his top left rib.
Injured was working on #10 locomotive, trouble-shooting an electrical problem, when he felt a pain in his back.
EE STEPPED IN A HOLE WHILE DRAGGING A TUBE AND TWISTED BACK AND GROIN. DIAGNOSED AS A STRAIN/SPRAIN. THE INJURED BEGAN MISSING ON 1/13/05.
The employee was changing batteries on a scoop and needed to climb on top to remove the battery plug. When the employee was climbing off the scoop, he slipped and bumped his right knee. The x-ray was negative, swelling and two days away from work.
Injured stated that while working loading supplies on bolter he felt a pull in his lower back. injured continued working until 4-18-05. injured is having surgery. didn't become reportable until 4-18-05.
EE WAS ASSISTING WITH THE INSTALLION OF A TOP TRAINING ROLLER ON THE SURFACE STACKER BELT WHEN HE EXPERIENCED PAIN IN HIS LOWER BACK. THIS INJURY OCCURRED 12/17/04, BUT HE DID NOT START TO LOSE TIME UNTIL THE END OF 12/04.
Employee was trying to remove drill steel that became fouled in hole, as he began to stand up he placed his right hand on drill head, the drill steel dislodged striking his right thumb causing a fracture and laceration.
EE HURT HIS RIGHT SHOULDER LIFTING WHEN DOING MAINTENANCE WORK. EE ASKED TO SEE COMPANY DOCTOR ON 12/23/04. EE WAS SENT FOR MRI ON 1/4/05. RESULT SHOWS DISK DAMAGE. COMPANY DOCTOR IS RECOMMENDING EE SEE SPECIALIST.
EMPLOYEE WAS PITCHING A ROCK WHEN IT BROKE IN HALF AND FELL ON THE TOP PART OF HIS LEFT FOOT.
Employee was trouble shooting a brake problem on the Cat Lowboy trailer. He began to loosen the bleeder plug to check for pressure. The bleeder plug blew out of the cylinder causing fluid to spray into the employee's eyes.
BENT DOWN TO LOOK AT TIPPING VALVE, WHEN STOOD BACK UP, KNEES GAVE OUT. BECAME MSHA REPORTABLE AFTER MRI RESULTS AND SURGERY WAS SCHEDULED 2/1/05
Employee was loading bulk trailer when driver removed chocks and stop sign. Employee jumped so he would not get crushed by ramp and hit the safety rail on trailer, causing a puncture wound to his right temple area and right thigh area. Employee received 3 to 4 stitches on each wound.
EE REPORTED ABDOMINAL PAIN ON 12/16/2004. EE CONTINUED TO WORK UNTIL EE VISITED A PHYSICAN ON 12/26/2004. EE STARTED MISSING 12/27/2004. EE WAS LOADING A ROOF BOLTER.
Employee was assisting with a 1 break pull back on 42"" belt. He picked up a top roller, walked about 10 feet, dropped the roller, went to his knees, and complained with a pain in his low back.
Operator was stepping from excavator track on to a pile of muck that was covered with snow and ice. Operator slipped when he put his weight on the snow covered muck pile twisting his left knee.

EE was operating CAT D25C articulated truck & was backing up to get loaded by the track hoe. EE was looking over his right shoulder to look up at the track hoe. EE stated that the weight of his hard hat & the positioning of his head caused him to strain his neck. He said that his truck was moving slow & the ground was rough. EE did not report neck strain until 2:30 pm.
The employee was cleaning under the conveyor of Plant II, a metal bar fell off the strap (conveyor) & bounced off EE's rt. chin causing a cut on the outside & inside of his lip. (Rt. mandible traumatic wound).
INJURED WAS MOVING CABLE WHEN RIB ROLLED STRIKING RIGHT LEG.
After carrying a new jack to work area he complained of numbness in his right arm.

EE WAS TURNING AROUND IN SHUTTLE CAR AND PUT FOOT OUT ON THE MINE FLOOR. WHEN THE MINER OPERATOR RAN INTO THE SHUTTLE CAR, EE FELT A PAIN IN RIGHT KNEE. MRI CONDUCTED AT THE END OF THE MONTH SHOWED THE EE NEEDS SURGERY AT A LATER DATE.
Employee was assisting co-worker to weld pipe together. He was not wearing safety glasses or safety shield to cover his eyes so he suffered welder's flash. He did not see a doctor but missed one day of work and then returned.

Employee opened a 1 1/2 inch compressed air line valve. Debris was blown from the line and avulsed the knuckle of his right index finger. Surgical debridement was performed and the employee was taken off work.

EMPLOYEE REPORTED: ATTEMPTING TO PULL CONTINUOUS MINER CABLE OVER TO PLACE ALONG COAL RIB. HE WAS WEARING RUBBER GLOVES FOR HAND PROTECTION. THE GLOVES WERE DAMP & THE NYLON CABLE STRAP WAS DAMP. WHEN PULLING THE CABLE, HE LOST HIS GRIP FALLING AGAINST THE COAL RIB. HE CAUGHT HIMSELF AGAINST THE RIB BY EXTENDING HIS RT. ARM OUT. SUDDEN JAR RESULTED IN PAIN TO HIS SHOULDER.
EMPLOYEE WAS ROOF BOLTING WHEN THE DRILL STEEL GOT STUCK IN THE ROOF. USING A PRYBAR TO PULL IT DOWN, THE BAR SLIPPED AND HE PINCHED HIS FINGERS BETWEEN THE PRYBAR AND THE MACHINE.
Employee was walking across a snow-covered roadway and slipped and fell on to the frozen ground. He finished working his shift and his regular shift the next day. He did not report to work on 12/17/04, resulting in a lost time. He suffered a back sprain due to this accident.

EE REPORTED ON 12/28/04 THAT HE WAS INJURED ON 12/14/04. WHILE LIFTING A MINE DOOR AT THE PORTAL, HE EXPERIENCED PAIN IN LOWER LEFT ABDOMEN. EE HAS NOT MISSED ANY WORK. NOTE: HAD SURGERY 1/10/05 & RETURNED 2/14/05, MISSED 25 DAYS.
Employee was refueling his dozer when he slipped and came down on his left knee. Contusion to knee at this point but has to follow up with a specialist.

OVERHEAD BUCKET OF CRANE HIT THE HOPPER EE WAS WELDING IN. A PIECE OF MARL FELL OFF OF THE TOP OF THE HOPPER AND INTO THE HOPPER, HITTING EE IN HIS RIBS (LEFT SIDE).
Employee was loading the roof bolter when he stepped on a rock causing him to twist his back. He has been diagnosed with a lumbar strain that has resulted in lost days.

DID NOT MISS WORK UNTIL 2/3/05. RETURN 2/8/05. WAS LIFTING FEEDER FLIGHT SHARP PAIN RIGHT SHOULDER.
EMPLOYEE TRIED TO OPEN THE DOOR TO THE BUILDING AND IT WAS FROZEN - HE KEPT PULLING ON THE HANDLE.
Subject had built a fire to get warm. He turned his back toward the fire and got too close and a spark caught his clothing on fire.
Employee alleges he struck his left knee on the frame of the scoop he was operating as he was getting out of the deck.
Employee was stepping off a lowboy trailer and lost his footing. As he fell he struck his left elbow on the trailer hitch. This resulted in a broken head of the radius requiring surgery to repair.
Bent over to pick up a tow cable and felt sharp pain in lower back. Was stuck in the mud at silt dump site and the cat came over with the tow cable to pull him out. 1"" cable dropped and when I bent over to pick it up off the ground I felt a sharp pain in the lower back.
Upon grabbing a water line, ee's finger was punctured by a piece of metal. The wound to the finger became infected.
Employee was cleaning out a hole at the base of his working face when a rock struck him on the back of the hard hat. This caused a laceration to the scalp that was sutured. The employee did not return to his next shift.
EMPLOYEE RECEIVED A ""BOIL"" ON THE LOWER BACK FROM THE LOADER SEAT. EMPLOYEE SAID THE SEAT IRRITATED THE SPOT. THE SPOT BECAME VERY TENDER.
He proceeded through the intersection, battery end first, turning left. When his trailer tire came to the corner he noticed it was going to rub the rib but continued to slide the tire around the corner which popped some of the rock off and shot it towards his operator's cab striking him in the forehead. Cut above his eyes required some stitches. Use protect safety glasses.
EE was running wire rope through new tail piece, when he hit a piece of sheetmetal that was leaning up, striking him directly behind his steel toe on his right foot. He was taken to hospital, x-rayed, nothing broken, but bruised up. Doctor told him to stay off of it for a couple of days.
The injured had put the snow-blower back into the storage barn after cleaning snow from the sidewalks and began walking across the parking lot to the main office. As he walked through the parking lot where a loader had plowed the snow, he slipped and fell on his right elbow. He had surgery to correct the injury on 4/5/05.
This is on injury that developed after a non-reportable fall 11-16-04 where the person caught his right foot in a tail pulley handle guard when he slipped and fell and at that time did not have injury and went back to work. his right foot toe dislocated now.

Employee stated he was helping lift and hang miner and loader cable when he felt pain in his lower back, close to his hip.

EMPLOYEE SLIPPED ON COAL WHEN HIS RT. FOOT SLID OUT FROM UNDER HIM & HE FELL ON HIS LT. KNEE & LEG.
Employee was jackleg bolting during shift and at the end of shift felt a sore left shoulder. Did not recall an injury or event which caused the shoulder pain. Diagnosed with a sore shoulder muscle. Missed three days. Returned 12/20/04

INJURED PERSON & ANOTHER MECHANIC WERE SLIDING A NEW HYD. CYLINDER INTO THE BOOM TUBE ON LW MULE. WHEN THE HYD. CYLINDER SLID INTO THE INJURED PERSON HE CAUGHT HIS RT. HAND BETWEEN BOOM FRAME & SLATE BAR THAT WAS BEING USED TO HOLD THE CYLINDER. FRACTURE TO FINGER ON RT. HAND.
EE WAS SHOVELING COAL ON A BELT. HE WAS IN A BOX CHECK AREA AND THREW A LUMP OF COAL ON THE BELT. IT HIT A CABLE AND BOUNCED BACK OUT AND LANDED ON EE'S FOOT.
Employee was carrying timber and shoveling coal and felt pain in right shoulder. Getting physical therapy for pain.
Employee was loading a belt for a belt move, while pulling the belt with channel locks employee felt a pain in lower back.
Haul Truck Driver recieved back injury while being loaded by a 992C loader. Loader bucket hit the side of the truck and jolted the employees back.
EE SLIPPED AND TWISTED ANKLE WHEN EE STEPPED OF THE PV WHILE UNLOADING A PUMP. LOST TIME INJURY
Employee experienced painful back spasms while operating an articulating haul truck while moving stripped material. Employee had experienced back spasms before and the case was put under investigation. As of 1/18/2005, the case was accepted as an injury that was reportable.

Employee scooping gob in order to get to pillars to mine. While scooping, a piece of 3"" plastic water line sprang & hit him in face resulting in cuts & bruises to nose & lips. He was taken to hospital, treated & released. Also knocked out 1 tooth.
EE exited the left side of cab. When reentering cab, EE placed left hand on door jam while slamming the door. He caught his left thumb between the door and the jam. He had 3 broken bones which required surgery to repair.
EE WAS CLIMBING DOWN THE LADDER OFF THE CRUSHER AFTER PUTTING GEAR OIL IN. HIS HANDS WERE SLIPPERY FROM THE OIL. HE SLIPPED AND FELL TO THE GROUND, LANDING ON HIS BUTT.
EE was setting timbers around the new power center area, when the corner rib rolled off striking him on the left ankle.
Injured, a miner operator, was pulling miner cable when he pulled muscle in back.
Employee was loading roof bolt supplies onto a roof bolter. He twisted his left knee while loading a box of glue. This did not become a Lost Time Accident until April 27, 2005.
THE DRIVER WAS HAULING COAL FROM DEEP MINE TO PLANT DUMP SITE & THE TRUCK GOT LOOSE. HE BECAME SCARED & JUMPED FROM THE TRUCK, BREAKING HIS LT. ANKLE & CUTTING HIS HEAD. THE TRUCK STOPPED IN THE DITCH.
While tramming the miner, ee was struck in top of the head by a piece of draw rock. Rock measured approx. 10'L by 12"" thick.
Employeee recieved whiplash on 9/19/03 while backing loader and looking over shoulder when front tire ran over 12"" diameter rock causing whiplash, but injury was not serious at time. He continued working for the past year and neck became increasingly painful. Injury detected in 12/04 in upper neck region that required surgery. He pre-op for surgery on 12/9/04.
EE ALLEGED - GETTING OUT OF DISTRIBUTION BOX HE TWISTED HIS KNEE. FIRST LOST TIME DAY WAS 12/16/04.
Employee was moving the miner from #7 to #6 face through the last open crosscut. A rock fell out from between the roof bolts striking his head and sliding down his back. The rock was approximately 3"" thick x 2' wide x 4' long.
EE WENT TO GET THE CHOCKS OUT OF THE BACK OF THE 019 MAN BUS. DOOR WAS LOCKED, SO EE WENT IN THE SIDE DOOR AND REACHED BACK TO UNLOCK THE DOORS. WHEN EXITING BACK THROUGH THE SIDE DOORS, STEPPED DOWN AND TWISTED RIGHT ANKLE
Employee was struck by a rib roll while setting timbers. He worked the next day after his injury. then he went back off on his injury and returned to work on dec 16, 2004.
Alleged accident happened when miner moved rock to hold drawbar down on pump.He told other that he thought he hurt his back.he finished week out w/o incident.He called on Monday and said he was going to family dr and she put him off work until 12/27. we sent him to our occupational Dr on 12/14 and he immediately release him for lighduty as of 12/20 he has not reported back to work.

While mining operations were being done in #8 entry of the 001 section, a piece of rock fell out between the rib and the first row of bolts. The rock hit the employee on the left side of his back and down to his left hip, causing him to fall forward. The rock measured 68"" long, 16"" on one end and 30"" wide on the other end, 2"" to 8"" thick.

An employee was roof bolting (with cobra drill) on the 21 west long recovery face, a piece of stone 28"" long by 7"" w by 8"" fell between the screen and the tip of shield, striking him on the top of his right hand. Fx metacarpal bone near ring finger.
EE WAS REPAIRING BOLTER CABLE THAT HAD BEEN CUT BY A CAR. AT POWER CENTER HE PUT HIS LOCK & TAG ON BOLTER CATHEAD AND SPLICED THE CABLE. WHEN FINISHED, HE TOOK HIS LOCK & TAG OFF & PUT THE CATHEAD BACK ON THE POWER CENTER. TRIED 3 OR 4 TIMES TO RESET THE BREAKER - WOULD NOT RESET, SO HE REACHED DOWN TO TAKE THE CATHEAD OFF WHEN THE BREAKER BLEW UP ON HIM.

EE WAS FLIPPING A ROCK ON THE TABLE. THE ROCK HAD A DRYCRACK IN IT AND BROKE INTO 2 PIECES. ONE PIECE FELL ON HIS LEFT FOOT. HIS FOOT IMMEDIATELY SWELLED & BECAME DIFFICULT TO WITHSTAND WEIGHT OR PRESSURE.
EE WAS GOING FROM CROSSCUT TO CROSSCUT GATHERING SUPPLIES. HE WAS DRIVING A 3-WHEEL MANTRIP. WHEN HE PULLED TOWARD THE INTAKE STOPPING, HE OBSERVED A ROLL OF BELT WITH A METAL TUBE STICKING OUT. HE LOADED SUPPLIES ON THE MANTRIP. WHEN HE STARTED TO BACKUP, HE FORGOT ABOUT THE ROLL OF BELT AND STRUCK HIS HAND AGAINST THE METAL TUBE STICKING OUT OF THE BELT.

Employee was roof bolting when he dropped his pot to put another piece of steel in and hit the wrong lever, causing the steel to hit his left index finger, lacerating it.
HE WAS CARRYING ROOF BOLTS THROUGH 22 CROSSCUT ALONG ME #1 BELT HIT HIS HEAD ON A ROOF STRAP JAMMING HIS NECK.
EE, a miner operator, was helping install a buggy anchor. After the anchor was installed, EE bent down to pick up a piece of key steel from the roof bolter drill boom. The center shuttle car took up slack on the cable one break inby, causing the anchor to strike EE in the chest. EE sustained a cracked sternum and is currently Lost Time.
Two employees were bolting at the face. When finised in one area, operator lowered the canopy and was suppling his tray with roof bolts for the next place. As he laid some roof bolt on the tray he bumped his elbow the canopy. He had small 1/4"" cut. On Saturday it became swollen and he was ill with fever and nausea. He was treated at ER, released; no missing time until 12/13/04.
Standing on trac of dozer urinating, his hip/leg gave out causing him to fall 6 feet to the ground. Employee was off work from 11/21 to 11/24/04 and again on 12/5/04 for a non-work related hip/leg injury.
Became reportable on 12/28/04 when medical report & MRI results were received. EE was walking when he twisted & turned his rt. knee, resulting in pain that continued to increase. Note: MRI showed no tear, but prior condition & degeneration.

Employee attempted to retrieve a large rock from the end of a 6-inch vacuum hose and caught his left arm in the hose. The power was shut off within 30-seconds by a helper nearby. Injury was limited to upper arm soft tissue injury/hematoma.

The employee was assisting in spotting a material trailer and got caught between the machine and the rib.
The day shift crew was heading outside at the end of his shift. They approached the outby set of track doors. The manbus had just about came to a stop when it unexpectedly started tramming and ran through the doors before the operator could bring it to a halt.
Bolter operator bolting up cap coal. When drill steel when through cap coal the coal dislodged fro the mine roof striking the operator on his head & shoulders.
THE INJURED WAS WALKING ACROSS A SHUTTLE CAR CABLE. WHEN THE CABLE REEL ON SHUTTLE CAR WAS ENGAGED, THE CABLE FLEW UP AND STRUCK THE INJURED KNOCKING HIM TO THE MINE FLOOR.
EE WAS DESCENDING THE SOUTH LOWER STAIRWAY FROM THE PRIMARY CRUSHER DECK. ABOUT THE THIRD STEP DOWN, BOTH FEET SLIPPED ON THE STEPS, LEAVING CONTACT WITH HIS HANDS ONLY. EE TIGHTENED HIS GRIP ON THE HANDRAILS TO PREVENT BODY CONTACT WITH THE STAIRS. IN DOING THIS, EE CLAIMS TO HAVE TWISTED HIS LOWER BACK.
EE & COWORKER WERE WORKING USING A CUTTING TORCH TO REMOVE SUPPORT PIPE. PIPE WAS HEAVIER THAN ANTICIPATED, SLIPPING THROUGH COWORKER'S GLOVES AND HITTING EE ON THE FOOT. (EE WAS HOLDING THE CUTTING TORCH. PIPE WEIGHED APPROX. 12 LBS.)
Co-worker was in a CAT 637E Scraper and recklessly hit the back end of EE's CAT 631 Scraper.
The employee was egressing from his work station (down the coal plant control tower stairway) to perform hopper cleaning duties. As he was coming down the stairway he twisted his knee and heard a pop in his left knee as he stepped down from the last step. His left knee was termed a strain at the medical facility and was returned back to work without restrictions.

Making a belt splice, the blade broke cutting his hand.
The employee sustained an injury to his right knee due as a result of slipping on a snow covered surface. The employee was walking from the administration building to the shop ready line.
Employee was lifting an angle iron and felt a pain in his groin area. Per call to Robert Biggans 2-2-05, hernia was repaired. coder 12.
Employee stepped on a rock (measuring 4""x4""x4"") that was in heavily rockdusted area, causing his right knee to twist. Employee continued working until 2/2/05 when surgery was performed to repair his knee. He started missing work at that time.
EE WAS HAULING SAND & GRAVEL WITH A 631E SCRAPER. EE WAS ENTERING THE STOCKPILE AREA AND ENCOUNTERED A ROUGH AREA. THE SCRAPER BOUNCED AND THE CAN DUG INTO THE GROUND AND PLOWED TO A STOP. THIS CAUSED LOWER BACK PAIN AND FRACTURE OF ONE VERTEBRAE COMPRESSION.
While unplugging the 1st stage cyclone, ee walked into hot material around where they were unplugging-resulting in 2nd degree burn to right ankle. ee diagnosed 12-6-04.
THE EMPLOYEE WAS IN THE BACK OF A PICK-UP TRUCK DISMANTLING A KENNEDY PANEL. HE WAS PULLING AND JERKING ON THE PANEL WHEN IT GAVE WAY. THE EMPLOYEE STEPPED BACK AND HIS LEG CAUGHT THE TAILGATE AND HE LOST HIS BALANCE AND FELL OUT OF THE TRUCK. **LOST TIME ACCIDENT** FIRST DAY OUT 12-2-04.
Worker was on his way to an area at the middle of his shift and drove over a drainage swale across a main road causing the loader to bounce and swerve. The operator hit his head on the window and frame on the right side of the cab. He was wearing his seat belt and aware of the drainage swale.
Employee was scaling a rock from the mine roof when it fell landing on his left foot. The falling rock caused mud to splatter in his eyes. While removing his foot from under the rock he twisted his knee. First day of work missed was on 12/5/04.
The employee was running a shuttle car when he hit a bump in the road and jammed his back. The accident was not immediately reported to the employer and medical help was not sought until 12/09/04.
EE was severely injured when his personal vehicle backed over the upper portion of his body while attempting to jump start it. In order to perform this task he had to place himself under the right front tire of the vehicle. Once contact was made, the vehicle rolled in reverse over EE.
Lost control of truck, ran in ditch & turned over. Laceration to right ear. Broke fender, air filter, door & mirror & damaged corner of truck bed.
Driller was loading empty barrels into the back of the work pickup truck. While in the process he felt a twinge in his lower back. Over the course of the next week and a half, his back started to get worse by the day. Finally he went to the doctor on 12/9/04 who said his back was strained and out of place so he popped his back and prescribed muscle relaxers.
Was replacing a hydralic hose on a D11 Dozer and stepped from the ripper onto the track. There was a globb of grease as he stepped. He slipped and fell off the dozer. At the time he said his back and hip didn't hurt as bad. It gradually got worse. He worked through December 4th and notified his supervisor on December 6th.

Haul truck operator was coming down the ramp from the primary crusher. It appears that he lost control of the vehicle and as a result, flipped over.
EE STATES HE WAS TRYING TO PULL A STUCK CRIB BLOCK OUR FROM UNDER A TRAILER AND FELT PAIN IN HIS LOWER ABDOMEN. HE WORKED LIGHT DUTY UNTIL 1/05/05 AT WHICH TIME HE HAD SURGERY
EE WAS TRYING TO PULL A CONDENSER OUT OF RETORT #3 IN CALCIUM FURNACE #8. HE GRABBED ONTO THE PIN OF THE CONDENSER WITH HIS HANDS AND WHEN HE PULLED THE CONDENSER IT CAME OUT EASIER THAN EXPECTED AND HE FELL BACKWARDS, LANDING ON HIS LEFT WRIST. HE WAS BROUGHT TO THE HOSPITAL AND DIAGNOSED WITH A FRACTURED LEFT WRIST.

The employee was attempting to climb onto the excavator machine when he slipped. His left shoulder impacted the track on the vehicle.

Bolter operator was on his knees operating roof bolter when a rock fell from the top & hit his back & then landed on his leg.
PSC Employee was hydroblasting roof structures using a CRS 15K Shotgun. He states that he released the trigger, and relaxed his grip on the gun expecting it to depressure. The gun pressure did not decrease, causing him to lose his grip. The gun got away from the EE and whipped around, directing the stream in an upward angle from left to right across his legs.
Employee slipped and fell on icy walkway, causing a pulled muscle in his lower back.
STONE FELL FROM CRANE WHILE UNLOADING, EE CARELESSNESS CAUSE OF ACCIDENT.
EE was using 16 lb. hammer on pipe and missed the lip on the 18"" pipe. EE caught his finger between the handle of the hammer and the edge of pipe. The handle then swung around after hitting the pipe and hit EE's left side of torso. Injuries resulted in a laceration to the right forefinger and contusion to left side torso.
LARGE ROCKS SMASHED CATWALK CAUSING INDENT.
While preparing to assist in removing chunks of ice from a flat conveyor discharge chute, the employee climbed three steps to the base of a conveyor catwalk, turned around, lost his footing and fell face-forward to the ground. He subsequently jammed his shoulder when he put his right arm out to break his fall.
While attempting to turn off a main haulroad in a service truck, the truck began to slide and eventually turned onto its right side. The employee was thrown across the cab and sustained injuries to his right thigh and knee. He began to loose time from work on January 3, 2005
Employee was assisting in removal of old feed pump at the #24 and 25 bird. pump shifted, employee attempted to balance himself but his foot became stuck in the mud and slurry resulting in twisted right knee. did not seek immediate attention. requested dr appt on 12/27/04. restricted duty began 12/27/04.
Lifting a bundle of roof bolt plates and placing them on the machine, employee hurt his hand, fracturing a bone behind the right index finger.
Packing and stacking powder bags of diatomaceous earth onto pallet. went to lift a empty pallet and position it for use. when temporary employee bent over at waist he could not easily raise himself into upright position.
On 12-21-04, injured filled out an accident report stating that as he attempted to pick up rub rail on M4 miner he felt pain in lower back. Injured continued working. On 12-28-04 injured called & said that he went to the ER on 12-24-04 & to the chiropractor 2 times since then. Injured returned to work on his next regular scheduled shift. Didn't become reportable till 12-24-04.

CABLES ARE DISCONNECTED AND ROLLED UP EACH TIME PUMP IS MOVED. EMPLOYEE WAS TREATED AS FIRST AID UNTIL PLACED ON RESTRICTED DUTY ON 1/31/05.
EMPLOYEE WAS LOOKING OVER OPENING TO DETERMINE HOW MUCH CLEANING WAS NEEDED WHILE MILL WAS OUT OF OPERATION. LOST HIS BALANCE AND FELL TO FLOOR. THE FALL RESULTED IN A FRACTURED PELVIS AND SOME BRUISES.
EE STRAINED BACK WHILE LIFTING WHEEL CHOCKS OUT OF THE BRACKET ON THE FRONT OF A HAUL TRUCK
EMPLOYEE WAS CLIMBING DOWN OFF A FRONT END LOADER AND FELL OFF THE BOTTOM STEP. WHEN HE TRIED TO CATCH HIMSELF WHILE FALLING HE LANDED ON HIS LEFT THUMB.
EE WAS REPLACING GRATING AFTER CHANGING SEPARATOR BLADES. GRATING SLIPPED OFF LIP, DROPPED AND JERKED EE RIGHT SHOULDER. REQUESTED DOCTOR VISIT ON 12/21 AND WAS PLACED ON RESTRICTED DUTY
Employee was replacing paddle hubs in blade mill. While lifting and moving hub on shaft he allegedly strained back. Employee went home at end of shift. He was transported from his home to the E.R. on the morning of December 16th, complaining of severe back pain. He was diagnosed with a strained back muscle.
The employee was hanging vent line while standing in the slightly elevated bucket of a loader. He jumped from the bucket (1 1/2 - 2 feet) and felt a pain in his back when he landed. He then drilled with a jackleg drill for about 2 hours and the following morning he experienced severe back pain.
Injured was carrying a set of drill steels to the roof bolting machine. As she attempted to step over a roll of screen that was staged in the entry in preparation for the longwall move, she caught her right foot on the screen causing her to stumble and injure her left knee and elbow.
EE was lifting a slab of sandstone up onto its edge to be split when he sustained a pain in his left lower abdomen and groin area. Doctor prescribed wearing of an abdominal binder support and modified work assignment for unspecified time frame.
Employee was stepping down off a low wall and overturned his right ankle.
Ee was bolting when drill steel bent as he was trying to start hole. He then hit wrong lever and steel rotated hitting right hand first aid was rendered and he worked rest of shift and the following day. Swelling persisted and he went to dr on 12/15/04. Diagnosed with fracture to first metercarpal. Released to restricted duty on 12/15/04.
Employee was tramming miner from #2 heading to belt heading, when cable had to be moved from path of miner. Employee bent over to move cable, his body hit tram levers causing miner to pin him against right rib in #2 heading.
Ee had been shoveling and when he started walking around he felt a pain in his groin and stomach area. Was taken to the doctor the following day where he was diagnosed with a hernia. Hernia had been repaired per call to Penni Carpenter on 2-3-05. coder 12.
Calcined rock was dumped from #5 calciner hot cyclone at 1:00 pm and cooled with water. After removing half the pile, steam was created during a typical lift when a hot pocket of material contacted water in bucket. Steam and hot material burned employee.
The crew of the production shaft were getting into the bucket for transportation to surface at the end of their shift. The last miner in the bucket entered facing inward, with his left arm rested on the rim of the bucket. The bucket belled up and before he could move his arm and his sleeve caught on the guard rail pulling his arm into a space between the rail and bucket.
The employee was filling the slinger duster from the air/dust tank underground. He was standing on the duster and had another employee turn off the dust and turn on the air to flush the line. The line surged and flipped up, causing the employee to loose his balance and fall off of the duster.
WHILE PLACING A 50 TON RAM UNDER THE CENTER PIN OF THE LOADER, EE FELT A STRAIN IN HIS GROIN.
While watching the material come off the conveyor and resting his hand on the rock box, employees hand was struck by a chunk of material, causing the injury.
Employee was working on brattices, he had finished one and started to work on another when a piece of draw rock (approx. 2'x2'x1/2-3""thick) fell out between the bolts and struck employee in the back. (Required stitches.)
THE MAINTENANCE EMPLOYEE EXPERIENCED A SHOULDER MUSCLE TEAR WHEN HE WAS ASSISTING A SECOND EMPLOYEE WITH MOVING A PIECE OF STEEL BAR 20' X 3/8"" X 6"" FROM THE SHOP STEEL RACK. AS THE EMPLOYEES WERE MOVING THE BAR, ONE EMPLOYEES CLOTHING SNAGGED ON A WORK BENCH CAUSING HIM TO TWIST AND DROP THE BAR. AS HE DROPPED THE BAR, HE DAMAGED HIS SHOULDER.
An employee was using a screw driver to apply force on two metal surfaces to seperate them. The screw driver slipped and under the force of her own weight, the employee fell forward and her face hit a part of the hopper below her face. The injury resulted in a cut and soft tissue damage.Stitches where applied by emergency physician.Pain and antibiotics where prescribed.
Wet soft sand conditions around scale created a vacuum on Larry's Boot. When Larry lost his balance his boot was stuck in the sand causing his ankle to be dislocated.
ASPER CITATION #7544826, EMPLOYEE ALLEGES THAT HE STRAINED HIS BACK LIFTING A SCOOP TIRE. WORKED UNTIL 12/13/04 THEN LIGHT DUTY.
PULLING ON STRUCTURE HE FELL, HITTING HIS SIDE ON ANOTHER PIECE OF STRUCTURE.
WHILE CARRYING A 30 LB PROPANE BOTTLE TO REFILL, THE EMPLOYEE SLIPPED AND TWISTED ANKLE. HAIRLING FRACTURE WAS DISCOVERED AT LATER DATE.
