Accident type, without injuries

Roof fall at station #7+80 entry #3. Fall measured 19.5' length x 19.5' wide x 6.5' thick. No equipment was involved in the fall. The area was cribbed & dangered off.

Damage to the north production shaft skip bat wing assembly unit was discovered by an outside employee at approximately 9:30 pm. The skip was removed from service until repairs could be made. The production shaft was down until Tuesday eve shift on 8/1/06.

Top fell approx 400' from longwall pull out, in #1 entry which is travelway to EP's behing longwall. Fall will be cribbed to allow firebosses to travel to EP's behind longwall. Length of fall 140' x width 18' x thickness 6' to 8'. Permanent roof support 6' torque tension bolts.

We had a roof fall occurred in the #7 entry in the intersection one break inby spad #39118. The fall was 50' long, 30' wide and 9 to 10 foot high. The area was bolted with 5' torque tension bolts. The fall was timbered and cribbed and dangered off. It will not be cleaned up.

Roof fall occurred at station spad 7833, #5 entry. It was bolted with 12'point anchors, we were setting cribs & 6"" beams when it started working again. We stopped the work & let it fall. 24'long x 18'wide x 12'high.

A roof fall (approx. 50' long X 70' wide X 0-14' thick) was found at start of 3rd shift. The HVAC cable and Water line were damaged during the fall.

A roof fall occurred at Spad 17365 in the No 4 Entry of the ""F"" Panel (#5 Section) drivage. Fall occurred in a 3 way intersection & was approx 50'L x 20'W x 7'H above the anchorage of the permanent support in this area. Timbers were set against permanent brattice to the south of the fall. Breaker rows of cribs were set on both ends of fall & walkway was established across the fall.

A roof fall was found outby spad #400 and inby #0393. for a distance of 60' in length x 19-20' wide and 4-7' high the date and time of fall is not known. the area was supruted with 48"" resin bolts and supplemented with cable bolts 12' a horseback was present.

The Cedar Creek Service Elevator was out of service on 7-31-06 from 9:00 pm until 11:15 pm due to slack in the braking system. Bagby Electric Company adjusted the braking system and the elevator was put back in service at 11:15 pm on 7-31-06.

A roof fall occurred on the A-mains section at #28 crosscut. The fall was approximately 20'x20'x8' thick. The area had been supported with 5' hight tech bolts. The fall was in the #4 entry.

At 2:37 pm a roof fall was discovered by the preshift examiner. The fall ranges from 36 to 48"" thick, 18' long, 19' wide. The fall will be cleaned up and supported. The location of the fall is at spad 222 in the #4 entry.

Roof fall in intersection #6 entry x-cut 77 5right main south. Rerouted primary escapeway set double row of breaker props around fall.

The tailgate of the 8 Right Longwall became blocked due to rock falling out between the crib line & the solid coal block. Location of longwall face was approx 8593 ft. Ventilation was not impaired & no injuries resulted from this blockage. Mining was temporarily stopped & implementation of the approved blocked tailgate plan was put into effect prior to resuming mining.

At approximately 1 minute after midnight the roof in the face of #1 to #2 entry x-cut fell. The fall was approximately one foot above the anchor zone of the bolts. The fall was approximately 17 ft wide, 8 ft high and 20 ft. long. The fall was part of an unbolted cut and two rows of supported roof. The roof bolters witnessed the fall from a distance.

Employee was using cutting torch to install safety hand rails. Hot material fell in tire pile and ignited tires. Hot work permit was in place.

On 7/28/06 an unintentional roof fall occurred on the 1st West Sub-Mains in #5 Entry at #12 Cross Cut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 18' long x 20' wide x 12' high and the roof fall will be cleaned up and resupported.

Had rock fall 2 breaks inby spad 5541 in right crosscut 90' inby rib to other intersection. There are other entries to travel out. Fall was approx. 20' wide, 60' long. Reported to MSHA.

An unintentional roof falal occurred 70 feet to the right of SS# 4712. The fall dimenions were approximately 50' L X 20' W X 10' High.

This area has been abandoned due to poor conditions. Fall is 30' long x 18' wide x 4' thick. Was bolted with 4' torque tension bolts. No injuries, no blockage.

A roof fall above the ankorage zone of the 48' resin bolts occured on the 001-0 NMU section in #3 entry 2nd open x-cut 90' inby spad #415 and 30' outby spad #415 the length was approx. 120' long X 19-20'winex 4-7' high the intersection and supplemental 12' cable bolts installed.

A roof fall was discovered at the #8 break on the 8N section. The fall was located in the crosscut between #1 and #2 and was 18 feet wide by 15 long and 5 to 6 feet high.

A roof fall was discovered in the 2 south #2 mains at 41 crosscut, #6 entry intersection. The fall measures approx 30' long, 18' wide and 6' thick. The area is supported with 5' fully grouted resin bolts and 10' point anchor bolts.

Rock fell in intersection only. Moved life line and reflectors around fall and dangered fall off. Timbers were set. Fall was approx 4' high. 4' brutter bolts were used. Length of fall was 18', width 18', thickness 4'. Nearest spad # 7182 one break outby. Fall did not affect ventilation and there were no injuries.

A roof fall occurred on # 2 section in # 3 entry at appromixately 4:50 PM. The roof fall measured 80' long 20' wide and 4' high. The area was supported with 48"" fully grouted resin bolts. There were no injuries. Fall was immediately timbered or cribbed off at all entrances.

A roof fall was discovered in the 2 South #2 at 38 Crosscut, #8 entry. The fall measures approximately 18 feet wide, 30 feet long and 10 feet thick. The area is supported with 5 foot fully grouted bolts dn 10 foot point anchor bolts.

An unintentional roof fall occurred at the New Era Mine at the 2E longwall beltline between crosscut 10 and 11, Entry #1 1525 East to 1675 East. The fall was approximately 30 feet long, approximately 18 feet wide and approximately 6 feet thick. The fall did not affect ventilation. The fall will be cleaned up. Fall #23K

An unintentional roof fall occurred in the intersection of the South West Mains in the # 7 entry, # 25 xc, 15+85. The cause of the roof fall was laminated and unconsolidated shale above the anchorage point. The area had 6' fully grouted rebar roof bolts for roof support.

#8 entry intersection 1 block outby spad #1657 fell 18' wide x 16' long by 5' high. The area had fallen out some on initial mining previously. The area was dangered off & cribbed. It will not be cleaned up.

An unintentional roof fall has occurred in the #5 intake entry at the #5 crosscut along the #3 belt panel. The fall was from rib to rib, approx 60 feet long and approx. 6 foot high.

The Enlow elevator was down for more than 30 minutes due to a bad hydraulic pump for releasing the braking system. There was no entrapment.

A roof fall was discovered the mine foreman in a normally untraveled entry. The location was in the #11 1st West Main, #7 entry, crosscut No. 2. The fall was well outby any working sections and measured aprocximately 20 x 20 x 6' high.

The Oakdale Portal elevator was inoperable from 1:35 am to 4:20 am. No entrapment, no injuries. Schindler's elevator service repaired faulty door latch. Elevator was returned to service at 4:30 am.

Spad #7504 #4 Hd. #4 Block: Bridge Unit (003-0) was on retreat mining cycle when some rock fell from Gob line on the ripper head of the continuous mining machine pushing the ripper head down in the bottom. Had to use the retriever to pull the miner out. No injuries & no damage to equipment.

On 7/24/06 at approx. 12:30pm the 1800 backhoe operator noticed smoke coming from the right front corner of the machine. The operator exited the cab to investigate. Upon discovering fire the operator returned to the cab, activated the fire suppression system and exited the machine. Attempts to control the fire were unsuccessful. Damage to the 1800 backhoe was significant.

Rock fall at NO.4 beltline, 14 breaks above No. 4 head drive. Between No.5 & 6 entry. Didn't fall on beltline. Fall was 30'long, 18-20' wide and 10'thick.

An unintentional roof fall was discovered H-12', width- 19'& 100' in length. Support- 5'resin (3/4 rod) & 10'cable bolts.

While Operator was mowing a dry hay field, a spark apparently was created between the hay mower and a rock in the hay field. Due to the very dry conditions, and the gusty Southeasterly winds, this caused the hay field to catch on fire. No equipment or personell was damaged or injured from the fire.

A reportable roof fall was found in 19 Tailgate, entry 3, crosscut 7. The fall was approximately 6 feet high, 15 feet wide and 16 feet in length.

A fall of ground occurred between shifts on the morning of 7/23/06. The fall of ground occurred in the south adit while rehabing old wooden timber sets. Approx 500 cubic yards of material came down when old timers failed.

Workers are normally off during weekends, but on Sunday, 7/23/2006, a special crew came in to repair our roofbolter. During the mine preshift inspection, the fireboss discovered a fall of ground in the intersection of #3 main and #3 crosscut. The accident was called in immediately to MSHA

The sugar run elevator became inoperative due to a 1/8 inch steel cable wearing in two. The cableis part of the open/close mechanism for the elevator doors. The cable was changed and the elevator returned to service @ 9:10am.

A roof fall was discovered on #2 unit, in #4 entry, at 62 crosscut. The fall measures approximately 30 feet long, 18 feet wide, and 7 feet high. The area is supported with 5 foot fully grouted resin bolts.

Timberland Hoist installation wiring was incorrect. Cooler for the gear box cooler control was wired to the #1 motor starter. This could cause the shut down of the other three motors. Wiring was corrected.

A roof fall measuring 30'long, 19'wide, 6'thick was discovered in the #1 return entry of out bound mains 20' inby spad 56261. Fall occurred due to laminated shale and rock streaks above the 5' fully resin roof bolts. Ventilation and the passage of men were not affected. No injuries or equipment was affected by a result of a fall.

An unintentional roof fall occurred at 24 break along the #4 underground belt conveyor in the #5 entry, size 60' Long X 7' high X 20' wide was supported with 4' fully grouted bolts with 10' cable bolts installed at intersections. No injuries, no equipment caught & ventilation was not interrupted. Fall will not be cleaned up. Roof fall was found during weekly examination.

A roof fall occurred on the 8N section at #31 break in the #2 entry. The fall is 15' long, 2.5 feetl thick to feather edges, and 16 feet wide.

An unintentional roof fall occurred on the 2nd main west at the 41xc, #3, 2 entries (24+60), the fall was approx. 135'long x 16.10'wide x 10'high. This area was laminated shell & was cribbed in the approaches due to adverse roof conditions.

A contractor and lafarge employee were installing a new belt scraper on the head section of a belt, and a piece of slag fell through one screen to get to another and landed on the bottom deck and ignited the polydeck screens. Minimal damage

An unintentional roof fall occurred on the 2nd Main West at the 43xc #3 entry (25+80). The fall was approximately 37' long, 16.9' wide and 10' high. This area had been cribbed in spots & narrowed down due to adverse roof conditions.

An unintentional roof fall occurred one crosscut outby SS# 3257 in the #5 entry. The dimensions were approximately 20'L x 20'W x 4'high.

A loop contactor switch went out on the hoist controls and had to be replaced. MSHA Newark field office was notified by phone. No miners had to be evacuated.

Build-up of fine clinker had accumulated in the clinker cooler. The clinker cooler baghouse was by-passed due to concern for overheating the baghouse cloth bags. The discharge gases heated up the insulation under the roof decking to the point that the insulation caught fire and smoldered. Fire was extinguished with no injuries and no major equipment damage.

An unintentional roof fall occurred in the #5 entry 8,400 feet outby the face ina neutral entry at Break #70. The fall measured approximately 20' Long x 20' Wide x 4 1/2' High.

Preshifter found rock fall in belt neutral at break 21. Fall measured approximately 120' long x 20' wide x 3'-6' high, it extended from mid point of #3 intersection to the right side of the crosscut between #1 and #2 entries.

On 7/20/06 an unintentional roof fall occurred on the #1 Unit, MMU-005, #7 return entry, #59 xc intersection. This intersection was supported with 10' point anchor roof bolts. The roof fall measured 18' long x 18' wide x 10' thick. The roof fall will not be cleaned up & all approaches have been cribbed off & flagged off.

A roof fall occurred at #1 1/2 wall, 3way intersection, #2 entry, C-6 off 1 North. The fall was above anchorage. There was no damage to equipment. The fall was 16'wide x 14'high and 20' in length.

The cedar Creek Service Elevator was out of service on 7-19-06 from 8:45 pm to 9:40 am due to electrical problems. The cedar creek elevator was also out of service on 7/20/06 from 1:30 am to 4:30 am due to door limit switch problem. Bag by elevator co. was called and repaired both items

Air shaft mains as on retreat mining cycle when draw rock & rock fell from gob area on the ripper head of the continuous mining machine-pushing the ripper head down in the bottom-had to use the retriever to pull miner out-no injuries & no equipment damage.

An unintentional roof fall occurred on the 1 South track haulage road at 26 Wall. The fall measures approx 70' Long X 16' Wide X 10'-12' High. No injuries or equipmetn damaged.

Fire reported by Cont. @ 7:50 am involving beltline & oil tank being salvaged. Safety arrived @ 8:20 am. Determined to be MSHA Rpt'l @ 8:25 am, called MSHA @ 8:35 am. Fire contained/extinguised, damage contained to belt & tank. Called MSHA @ 1:35p.m. to report fire out. Surface being salvaged by JB Gordon Construction.

A roof fall was found in the 5 west panel at crosscut #39 in the #3 entry outby #3 seal. The fall was approx. 8' high x 20' wide x 50' long. 5' high tech bolts supplemented with 8' super bolts and been installed in the area.

An unintentional roof fall occurred in the 003 section, #7 entry intersection one break inby Survey Station #2688. The fall measured approximately 30' Long x 18' Wide x 3' - 4' High.

Main Portal Elevator would not level on bottom landing. Repairs were made and elevator was put back in service at 3:55 p.m.

A roof fall occurred in the 3 East Panel in the intersection of #4 entry at cross-cut #85 measuring approximately 50ft. x 19ft. x 5ft. This area was supported by 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

At approx 5:15 am on 7/18/06 The roof in 7th left at crosscut 55 between No 2 & 3 entry began to fall. At 5:30 the roof was still falling and continued to fall as a dribble. At 6:30 am the fall was above the anchorage zone. The roof fall 14 ft wide 16 ft long and 8 feet high. The area was being rebolted with 12in cable bolts when it sarted to fall.

A roof fall 20'x20' undetermined height was found in an outby area at station 2851. The fall does not impede travel or ventilation.

An unintentional roof fall occurred on the 2nd main west at the 67 xc (40 + 20) in the #7 entry, the fall was approx. 60' long x 17' wide and approx 10' high. Cribs had been built in the approaches to the intersection on 6-30-06 due to suspect roof conditions.

Main breaker in sub-station tripped. When they first tried to reset breaker, it wouldn't stay in. Hoist went down at 9am. Msha was notified at 9:30am. Hoist was down and came back up at 3:30pm

A roof fall was discovered near spad 736, the approx size of the roof fall is 8' high, 70' long and 18' wide. No injuries were reported as a result of this roof fall.

North skip got hung up. Operator didn't realize North skip was hung up & was bringing South skip up. After spooling off approx 100' of cable on North skip, operator realized what was happening & stopped hoist & set brakes. After a while, the South skip started creeping downward & then took off setting South skip down hard & pulling North one through the limits. Damaged skips.

An unplanned roof fall occured 20' Inby spad No. 1351 in the #3 Entry. The fall measured 70' long, 20' wide and 4' to 6' thick.

Unintentional roof fall 3ssh #6 entry belt entry xcut #2. 30' long x 18' wide x 8' high. Bolted with 6' & 8' tension rebar bolts cribs on inby rib. Will be cleaned and bolted. Was developed in Sept 2005.

A roof fall occurred in the main east, #1 entry, #41 crosscut. The fall was approx. 25'long x 25'wide x 8'tall. The area was bolted with 6' + 8' point anchor bolts. The fall didn't affect ventilation, travel, cause any injuries or damage any equipment.

# 2 Service Hoist was inoperative from 4pm until 8pm on Monday the 17th of July. The fault was a digital input card used in the controls. No damage to the hoisting equipment was done. The card was replaced and the hoist has operated normally to date.

The typro creek service elevator was out of service on 7/17/06 from 10:47 pm until 7-18-06 at 1:40 am due to slack in braking system. Bag by elevator co. adjusted the braking system and then elevator was pit pack in service at 1:40 am on 7-18-06.

An unintentional roof fall occurred in #6 entry travelway between 102 and 103 xcut 2nd main east. 15' long x 16' wide x 7' high. Bolted with 6' tension rebar bolts. Will be cleaned up. Date of development 8/30/93.

4160x480 volt transformer secondary lug got hot. Wire burnt off and blew primary fuse; that in turn caused a surge in the system and faulted out the PLC chassis rack. This caused the PLC processor to fault. When spare processer was installed it faulted due to bad chassis rack, the crew was then brought out by manually operating work deck winch starters.

Coal was being mined int he face of the north entry 80' inby survey staiton 638 on the 2 left off north mains section. When the cutter bits of the continuous mining machine struck sanstop the machine operator saw an orange flash.

While cleaning the coal seam a dozer contacted the casing of a plugged gas well in the DII Area pit. Pressurized gas started blowing from the well. The dozer was backed away from the well and the area was evacuated. THERE WAS NO FIRE. It was determined the upper well plug was incorrectly located at the top of the coal seam instead of below the seam.

There was a rib failure in the #54 ramp. It broke past the anchorage of the split set bolts about 6"" to 12"" in an area about 18"" in length. No one was working in the area. The failure didn't go into the back, didn't impede ventilation, didn't impede foot traffic. Safety foreman notified Boise Field Office when he found out about the failure 1st thing Monday morning 7/17/06.

Employee deliberately disobeyed warnings to cease welding on inflated floatation tire rim ten minutes before tire blew up. Heat from welding plus compression of air in tire equals massive explosion.

Contact with electrical current

Employee of Adams Trucking was weedwacking brush. Blade on weedwacker hit guide wire for electrical pole. The wire was severed and the tension pulled it back toward the transformers on the pole. the guide wire came in contact with a bare wire. At this time we assume electricity went to ground causing fatality. Under Investigation.

Employee was re-bolting on the 2 Main North supply road (#6 entry) at crosscut #24 when a diesel scoop with a duster attached to the bucket struck employee on the lower part of the right leg m ashing it between a metal brace on the duster and a metal bracket on the bolter. The leg was partially severed between the ankle and knee. Doctors made the decision to amputate completely.

EE was assisting other crew members string a rock crusher conveyor belt when his left ring finger got caught in a v-belt sheave resulting in partial amputation.

Excavator overheated in 90 degree plus heat. Employee attempted to cool off excavator by spraying it with a hose. The radiator cap exploded spraying him with hot antifreeze.

Employee was changing out a feed cable on the boom of a Tamrock bolter. The bolter was plugged into a power source. The trailing cable was energized from a distance by an electrician who could not see the bolter. The plug from the disconnect had one phase to ground. When the feed cable was energized, the mechanic was in connect with the bolter and recieved a shock.

Employee was roofbolting in a low area. The roof bolt bent and when employee pushed the bolt into the hole and immediately felt a sharp pain in his lower back.

EE was struck by a piece of broken face conveyor flight while observing the Headgate Drive sprocket, E-14 Longwall Section. The face conveyor chain broke, causing a piece of broken flight to be thrown , catching his left forearm between the piece of flight and the coal rib, resulting in a compound fracture to the left forearm.

Struck against a moving object

Employee was driving Mack dumptruck downhill. He lost brake pressure while trying to downshift. The employee tried to manuever the truck around a corner and the truck tipped and fell over on the driver's side.

Fall to the walkway or working surface

Slipped and fell on left arm.

Employee was operating an excavator; he swung the machine to the right and struck a rock. The machine came to a sudden stop. The operator then complained of neck strain and headache.
