burning pallets day before and wind created spark starting fire.
Sometime between 8/30/06 & 8/31/06 a fall occured in the return airway entry #2, section #3, 80' inby spad 17909. Fall was 60' x 19' x 8'. Both MSHA and OMSL were contacted and investigated the area. Area is to be rehabilitated, will install additional support as needed. Both MSHA and OMSL were notified immediately.
A roof fall was discovered in # 7 entry 1 break inby spad 647 the estimated size of the roof fall is 6ft high 18 ft wide and 50ft long. No injuries where reported as a result of this roof fall.
Elevator stalled between floors.
A rock fall occurred at King Block in the track entry. Fall measured approx. 18' wide x 20' long x 5' high.
A roof fall occurred along the Main South roadway between crosscuts 67 and 68. The cause of the fall was moisture from roadway air which caused the top to cut and rash.

An unintentional roof fall was discovered on 8/31/06 during a weekly examination. The incident location was at the #6 entry approximately 6,500 ft outby the face at Break #95, Spad #1331. The fall measured approximately 120 ft long x 20 ft wide x 4 1/2 ft high.
Rock fall was in 3rd East Intake, #2 Entry, approx. 6' high by 20' x 22'. All approaches were timbered. Fall did not affect ventilation or block escapeway.
A roof fall occured on #3 unit #9 entry at crosscut 9 spad (5+45). The fall is approxmately 20' long x 18' wide x 5+'

A roof fall measuring 60'L x 20W x 7H occured in the No. 6 entry off 003 section (005 MMU) 2 breaks inby spad No. 8054 in the Panel 1 submains. Area was cribbed and dangered off.
A roof fall occurred in the 2nd west submain, #4 entry, #3 crosscut. The fall was approximately 18' long, 18' wide and 8' tall. The area was bolted with 6' & 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.
A roof fall on 004 beltline panel approx 18'w x 25'l x 5-6'h. Some moisture and slip appeared to be the cause of the roof fall. The area will be cleaned up and resupported with longer point anchor bolts.
X-cut 56 7th 48 (Entry #6) old #4 Mains. Length 30', width 40', thickness 8'.
A roof fall measuring 70' long 18' wide 6' thick was found in the return air course 11,000' from the active section. The area developed March 97 with 4' resin roof bolts. Laminated coal/rock streaks above the anchorage zone caused the fall. It did not impede travel, disrupt ventilation, and no injuries occurred. Entrances to the fall was barricaded and resupported with jacks.
Unintentional roof fall 37 x cut north macas. #4-#5 entry. Fall measured 50'x 19'x 12.

Fire discovered and fought until3:40 P.M. exited building contacting Seneca County Sheriff dept. Fire Dept arrived at 4:07 P.M. fire extinguished at 4:45 p.m.
The shearer was operating at number 38 shield, cutting toward the tailgate. The headgate shearer operator was washing the toes of the shields when he reported seeing a small orangle glow. He immediately pointed the hose towards the shearer drum and stated that the glow lasted about 2 seconds.

#3 Entry inby Spad 699 a roof fall was discovered. The estimated size of the roof fall is 7 ft high 18 ft wide 50 ft long. No injuries were reported as a result of this roof fall.
A roof fall occurred on the 1 Left 4 West 2 Right panel. The cause of the fall was weak laminated shale. The fall occurred in entry 3 at crosscut 11. The fall is located in the entry adjacent to the 4B belt.
A roof fall was discovered in #4 entry 1 break inby spad 480 the estimated size of roof fall is 6ft high 18ft wide and 40ft long. No injuries where reported as a result of this roof fall.
A welder was burning a hole in a brace on a scoop battery in order to place a bolt in a c chanel that protects the battery from damage. Sparks from the torch ignited a pocket of hydrogen under the battery lids resulting in a small flame & pop.
While operating a torch cutting metal on the 4th level, the workmen ignited a small methane bleeder in the concrete wall 7 feet above the floor level. The flame was approx. 2 inches in length, burned 5 to 7 seconds, and was extinguished by using a gloved finger to smother out the flame.
An unintentional roof fall was discovered on the #1 section, #6 right entry to #7 entry (face) 1 block inby #4936 in #6 entry. The fall measured 40 ft L x 18' wide x 6 1/2 ft high.
A roof fall occured in the #5 entry of west mains at survey station #703. The fall was 1 break outby the section power center and measured approx 20' long x 20' wide x 6' high. There were no injuries or equipment involved. Ventilation was not affected.
the 10 west belt burned into due to slippage on the head drive roller. the tailpiece had been bumped during the loading process causing the tail roller to become gobbed. this in turn caused the belt to begin to slip on the head drive roller.
An unintentional roof fall was discovered on 4 North mains, #3 entry brk. 84 at spad# 18128. The fall measured approx 20' wide x 20' long and 10' high. This fall will not be cleaned up. No one was injured, no equipment was involved and ventilation was not impaired.
While conducting the fourteen day inspection of the emergency hoist, the wire rope cable broke allowing the basket to fall
The cap rock just outby the longwall tailgate fell in, restricting movement off the longwall face. The Tailgate Closure plan went into effect immediately. All miners were informed.
An ignition of methane occurred in the underground bin area during the demolition of the facility. Methane which was liberating through the joint in the wall and floor ignited when cutting torch blew sparks and slag against the location. Flame duration was 5 to 7 seconds and was extinguished by smothering it out. No injuries occurred.
While in the supply yard the hoist car became inoperative due to the shorting of the control circuit when two wires in the breaker box loosened. The wiring was repaired and the car was put back in service.
The timberland hoist (Minicage) at the production shaft hung in route to the bottom during a test run. One of the safety catches set which stopped the hoist in the shaft. The safety catch was repaired on 8/31/06 and hoist is operational. No persons were on hoist no injuries, no equip damaged.Hoist was removed from service until repairs were made.
While pushing refuse into the impoundment, the dozer operator went about 5 feet beyond the edge of the water, causing the embankment to fail resulting in the dozer being engulfed in the fine wet materials.
A rock fall occurred on 1 East Mains at break #49 on 4 track. Fall measured approx. 4' wide x 20' long x 4' thick.
Unintentional roof fall 3rd north sub main #4 entry 32 xcut 4-way intersection. Mined on April 7th 2003. 30' long, 22' wide, 9' high. Bolted with 8' pins tension rebar. Proped off.
A roof fall occured in the 1ST northwest mains #3 entry crosscut 160 to just inby crosscut 161. The fall is approxmately 110' long, 18.5' wide and 6'-10 high.
Fall app 0 to 7' high 18' wide, 50' long no injuries no equipment involved no blockage of of escape way or ventilation, will not be cleaned up breakered off & danger & plotted on mine map. Bolted with 5' resin bolts & 8' cable bolts.
A roof fall occured in the 15 north panel, inby crosscut #12 extending through the pillar line between crosscut #13 and #14 in the #7 entry. The fall measured approximately 75'X19'X5'-9'. The area was supported with 5' fully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.
A rock fall occurred in the first hand cut in the #4 pillar block in #5 heading 100' inby SS #7437. The continuous miner was stuck by a rock that fell on the miner head in the first cut; no roof bolts pulled or fall above anchorage. The cut was approximately 20-25', miner was stuck by 3-5' rock. No ventilation was disturbed, no injury occurred and the fall will not be cleaned up.
A roof fall was discovered in the # 5 entry at #21 crosscut of the 3 North Intake. Thae fall was approximately 20' wide, 20' long and 5' high. The area was originally bolted with 5' high tech bolts.
A roof fall occured at 39 block, on #11 belt, adjacent to the 3 west haulage. The fall measured 20' long, 10-12' high and 15' wide. No injuries. Area was originally mined in May of 1989 and bolted with 8' bolts.
Groundfall at the 4080 level. MSHA contacted and inspected.
A fall of ground (approx. 10'wide x 10' long x 3' thick) occured at the intersection of the A-4 sump and 900 roadway. Our Geology dept. has concluded the area contained a mixture of striped coarse and fine grained dolomite with a layer of calcite on the bedding contact. No injuries The affected area has been scaled and further ground support needs are under evaluation.
Unintentional roof fall 2nd West Panel #12 Entry 1260' mark. 20' long, 8' wide, 4' high. Advancing at 3-way intersection. Bolted with 3' conventional bolts. Area proped off mined on 8/18/06.
The tailgate of the 8 Right Longwall became blocked due to rock falling out betweenthe crib line and the solid coal block. The location of the longwall face was apprx 7230'. Ventilation was not impaired and no injuries rsulted from this blockage. Mining was temporarily stopped and the Blaocked tailgate Plan was implemented prior to resuming mining. At 2:45PM, tailgate reopened.
A roof fall measuring approximately 4' thick, 12' wide, and 18' long occurred in the intersection of the no. 2 entry 1 break inby survey station no. 731. Fall location is approximately 500' outby working section.
A fall of roof occured int he intersection of #27 crosscut, #1 entry of the west mains section. Fall was 6 ft high, 12 ft long x 14 ft wide. Roof rock was laminated with clay and mud seams with water present. Roof had been bolted to plan using 6 ft tension/rebar fully grouted oblts. Screen installed and ribs bolted. No injuries or equipment involved. Fall area was rebolted.
ROOF FALL (7000-1-008-06) - LOCATION - Main #4 Belt, Crosscut #102. DIMENSIONS - H:6', W:19', L:40'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' fully grouted bolts.
A conveyor belt support chain broke causing the belt structure to fall striking and breaking a 4"" water and 6"" waterline. The water caused debris to be washed down dip damaging two ventilation structures that separated the belt aircourse from the escapway. This incident may or may not meet the original intent of 30 CFR 50.10 & 52.2(h).
A roof fall was discovered in the 2 North 5 Mains, 3 crosscut, #5 entry. The fall measures approximately 20 feet long, 18 feet wide, and 10 feet thick. The area is supported with 5 foot fully grouted resin bolts.

Elevator car would not move off bottom. Problem was repaired and Alimak was put back in service at 8:00 p.m.

An unintentional roof fall occurred at 48 break along the #5 underground belt conveyor in the #7 entry, size 18'Lx4'Hx20'W. Was supported with 4' fully grouted bolts. No injuries resulted from this fall. Roof fall will be cleaned up. Roof fall was found during the weekly examination.
While scaling intersection of NW10-NWD, roof fell on boom causing machine to tip forward damaging right front axle housing, machine was working under bolted back, no damage to operator's cab or operator.(roof fall on scaler boom. per 220062580017)
A roof fall occurred measuring 6-8' high in the #2 heading #1 pillar block 40' inby SS 7544 last right hand cut in #1 block rock had fallen in cut only when attempting to move MRS out 2 rows of bolts fell out above anchorage. Equipment involved was an MRS, no ventilation was disturbed, no injury occurred and the fall will not be cleaned up.
Employee was operating the CASE Excavator when he noticed what he thought was dust coming from the engine compartment. Upon inspection it was discovered that a fire had started in the compartment. He exited the excavator and called for assistance. He was not physically injured in the incident.
An unintentional roof fall occurred at the New Future Mine at the 5N Headgate #2 Entry at Station 4496N. The fall was approximately 34 feet long, approximately 18 feet wide and approximately 12-15 feet thick. The fall did not affect ventilation or passage of men. The fall will not be cleaned up.

On August 21,2006, an unplanned roof fall occured ont he #1 section. #8 entry and 4,650' outby section. The length of fall was 50' width 18' height 4 1/2'. Will move return to other entry.
Unintentional roof fall 40'(L) x 18'(W) x 3.5'(T). The fall was on 1 West Mains, 6 breaks outby intake overcast on 2 Belt.
A roof fall occurred on the Main North Track outby #2 Portal at 112 xcut. (#4 Entry.) Fall was 22'L, 20'W & 7'H. Area was mined in Nov. of 1991. Fall occurred due to deterioration of immediate roof around supports (roof bolts). Area was supported with 8 feet long point anchor roof bolts with an expander shell & 2 feet of resin. Header blocks, screen & wooden plank were used.
A grass fire discovered at 7:45 p.m. on the mine. A powerline broke and fell to the ground, igniting the grass. The Fire was extinguished by Jewett Fire Department at 8:05 p.m. Some logs were smoldering but the active burning was out. The fire was reported at 8:16 p.m. because of the the smoldering logs.
The #1 entry, last open break intersection. Roof fall approximately 20' wide, 25' long. Was cribbed up and dangered off and called MSHA.
Unintentional roof fall #48 Xcut main east between #6 and #7 entry. 12'long x 14' wide x 4' high. Bolted with 6' tension rebar bolts. Called recorder. Didn't fall above anchorage, should not have been called in. Marked out and propped off area.
6 North track entry @ 80 Blk Intersection. Above anchorage x 8'-9' high. 1 block inby track overcast.
An unintentional roof fall occurred approximately 100' inby SS# 3405 in the #5 entry of the 003 mmu retreat mining section. The dimensions of the fall were approxiamtely 7'h x 15'w x 20' L.
A roof fall was discovered measuring 19'x19'x0-6', 70' inby survey station #1592 in the mains left return. No equipment was involved, no ventilation was disturbed, no injury occurred and fall will not be cleaned up.
An unintentional fall occurred in the #8 main north entry at the 19+40', #27 crosscut due to deterioration of the roof above the bolt anchorage. The area was bolted with 6' tension rebar bolts. Intersection will be timbered off.
During a normal weight test, the man cage became inoperable. A mine evacuation was initiated. No injuries or personnel in danger.
Roof fall occurred starting 1,200 ft inby #2 portal belt entry. The fall was approx 20 ft wide, 40 ft long, 7 ft high in between the coal blocks.

As truck was dumping a load of coal, the trailer turned over spilling it's contents.
Fire lasting longer than 30 mins. after discovery burned grass, HDPE pipe, and power poles near CIC. No injuries, no environmental release, no cyanide issues & no outside resources needed. Fire was caused by arcing on a jumper insulator on a line pole in an area where vegetation is dry. 3 water trucks responded ( 1 put out fire, 1 on hot spots, and 1 on stand by).
An unintentional roof fall occurred at spad #16+36 on the 4th N.E. submain #2 unit #5 #6 #7 engtry. The fall measured 21'x20'x10. It will be cleaned up.
8:00 AM a fall occured on 001 section in #4 entry out-by at spad #18921 in the neutral. No equipment was involved, air was not affected, fall was 10' long, 20' wide, & 4' high. Area will be cleaned and additional support installed. MSHA and OMSL investigated the accident.
A roof fall occured at the 1st North - Main East, Entry #5, Crosscut #76. The fall measured approximately 25' long x 16' wide x +9' thick. The fall did not result in any damage to equipment or property, but did affect passage of men.
Section C-9. On 8/17/06, an unintentional roof fall occurred on the 2nd 42 South haul road, in #6 entry between #61 to #62 Cross Cut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 50' long x 18' wide x 10' high and the roof fall will be cleaned up and resupported.
An untentional roof fall occurred XC 2-3 Bailey Bottom area near rock dust borehole. The fall measured approx. 60'L x 8'-10' H no injuries & no equipment damaged.
A roof fall occurred on the 1 Left 5 West panel in entry 36 at crosscut 20. The cause of the fall was weak laminated shale commonly referred to as stack rock. The fall was located in the primary escapeway inby the section loading point.
Apparently the driver attempted to down shift to a lower gear while descending a grade between road marker 4 & 5 on the haul road. He lost control in a curve & traveled through a berm impacting the hillside resulting in no apparent injuries to the operator.
A roof fall occurred on the MMU-002 working section, between entry 2 to 3 in the last open crosscut. The fall continued to fall and progressed into the intersection of entry 3 and traveled inby in entry 3 toward the face. Due to these adverse conditions, it was decided that the unit was to be moved to a different location.
A fall outby the face 10.500ft in the intake escaping when roof intersection in #4 entry 1 x cut inby spad #2854 back and falling. this area was supported with 6 in turpike fulley connected with 5 cable bolts in a stay pattern cribs & jacks were installed 20x30x15.
A roof fall was discovered during a weekly exam in the 1 West #2, 62 Crosscut, #2 entry. The fall measures approximately 30 feet long, 18 feet wide adn 6 feet thick. The area is supported with 5 foot fully grouted resin bolts.

The 8 haulage elevator was down for 48 minutes. The problem was a bad limit switch at the bottom of the elevator.

An unplanned roof fall occured in #130 mine, #7 entry, #4 belt at 14 break near spad 4125. The fall measured 40'in length, 20' in width and 5' in height.
a methane gas ignition has occurred in the 3 main north between spad 10+40 and 11+6. no injuries or damage to equipment was involved. a scoop bucket was being used as a welding platform. the hinge on a door that was to be installed at a latter time was being welded when the ignition occurred.
""O"" entry of North B Mains - fall of roof approx. 30' long, 28' wide & 8' high.
A roof fall was discovered on the #4 entry (track) on the North Mains. The fall measured 40' long, 19' wide, and 4.5' thick and occurred due to laminated coal and shale streaks above the 48"" full grouted roof bolts. Ventilation and the travel of men were not affected, no injuries or equipment damage occurred as a result of the fall. Fall was 3500' from the working section.
Size of fall: length:20'; width:19'; height:5'. 48"" resin. No cribs or timbers installed prior to fall. Type of strata: shale, mixed, laminated. No rider seam present. All entrances breakered off. Area will not be rehabilitated.

Roof fall was found at the 5001' of the #3 entry north at 2:10pm. Fall was in a 4 way intersection. Fall is 35-40' long, 7-8' high and 18-19' wide. Fall has return for #2 entry blocked. Fall will be cleaned up. Areas have been timbered off and cribs built on south end of fall. 6' tension rebar was primary roof support.

Both MRS's were recovered safely. This occured in the 1st left panel off of 1 south, in the #4 entry, at spad #970 at the #43 lift.
A roof fall was discovered at break 37 on I track entry inby spad 5111, the estimated size of the roof fall is 5 ft high, 19 ft width, 50 ft long. No injuries were reported as a result of this roof fall.

The #8 elevator was not operable for a period exceeding 30 minutes due to a malfunction of the top leveling switch, the switch was changed & the elevator was restored to service
An employee was leaving the mine level to travel to surface on elevator. Shortly after the elevator started up it stopped due to a fault on bottom door latch. Employee got off elevator at 8:18 pm

An unintentional roof fall was discovered 8/14/2006, at break #72,spad #1084. The fall occured 8,200 ft. outby the face in the #5 neutral entry. The roof was a mixture of sand stone and slate. The fall measured 16 ft. wide x 17 ft. long x 2-4 ft. high.

An unintentional roof fall has occurred on the 1st Main South in the #9 entry 18+00-17+40. The fall was approx. 80' long, 15.5' wide and 12' high. This fall occurred on an idle shift and was discovered by an examiner doing a fire check. The fall was called in at 1 pm Sunday 8/13/06.
On 8/14/06 at approx 8:10 am, while servicing the fan, a change in the mine pressure was recorded on Fan Chart and water gauge was noticed. Upon investigation, a roof fall in an intersection 3 X-Cuts from fan was discovered. This fall measures 18' x 18' x 8' and fall partially obstructs air flow and impedes travel of air course. The fan portal is located in the #5 Entry.
PRB Stockpile on upriver # 4 feeder area experienced a fire. The stockpile fire was close enough to the draw off tunnel feeder belt to ignite the belt. The coal pile fire, along with the feeder belt, were extinguished at approximately 10:10 PM on 8/13/06 by local fire department. There were no injuries as a result of this incident.

An unplanned roof fall occured 6000' from the active face of the # 1 section in the #4 entry at spad #4283. Fall measured 30' length 18' width x 6' heightno equipment was involved and ventilation and passage of persons was not affected . No accidents were reported due to this accident.
The 3 left marker section mined into the old 1 left area of the oad R&B mining corporations mine No.1 The cut thru was made in accordance with an approved plan. Once the cut-thru was made it was discovered that there was a fall in the old R&B mining corporation mine. The extent of the fall has not been detrtmined.
Mine examiner found the rock fall at break 31 of 1 East Mains in #5 entry extending over to the #6 entry. Fall measured approx. 20' wide x 120' long x 5' thick.
It was discovered there was a problem with the bottom switch for the Koepe access cage. The mine was evacuated via the production hoist. The switch was replaced and the Koepe became operable, after testing, about 12:50pm. No employees were in danger.
The roof in cross-cut #26 of B-Access fell approx 17' wide x 8' high. This fall is a continuation of another fall. The area is timbered off on all accessible sides. The fall was above the anchor zone.

A rockfall occurred on the 2C belt. The fall was 40 x 19 x 5. Five foot glue bolts were used in this area. The area was advanced on 70 x 50s. There was a hidden slip - no water in area.
On Friday 8/11/06, there was an electrical fault between the Norberg Hoist and the Fronteir Kemper Sanford Day Brake Car. The mother board was replaced. Both MSHA and state agencies were notified timely.

An unintentional roof fall occured at the #10 crosscut in #4 entry locaated in the 4-B intake air course. The area was initially supported with 6' point anchor roof bolts, the fall measured 65' x 18' x 8' high and the roof fall will not be cleaned up.
A hydraulic fitting on the hoist braking system cracked and leaked hydraulic oil. The hoist was shut down to install replacement fitting. No one was underground whle the hoist was down. Incident reported to MSHA Inspector Dustin Crelly who was on-site.
Crocker Reclamation balling hay H-1 reclamation. Bearing overheating cause fire on baler. He extinguished baler fire, and began estinguishing grass fire in windrow. The baler reignited, and ee turned to the tractr into the wind, i.e. out of the grass fire path and extinguished the baler again. Fire brigade controled the grass fire within 40 min. and 3-5 acreas of grass burned.

2 miners on a raise climber broke the bullhose. While descending they stopped, reason unknown. They tried to move turning the top flywheel only. A mechanic in the nest took the spare climber to repair the hose. That climber had a bad motor and returned to the nest. The mechanic went to bring the dayshift crew up to the nest, fixed spare, fixed climber in raise and both descended.
The intersection at #195 block just inby spad # 1657 fell. 18' long x 18' wide x 7' high. It was bolted with 5/8 x 72"" full grouted rebar bolts with 6"" x 6"" plates & 2"" x 6"" x 16"" boards. Bad geology & subsidence caused this fall. It is permanently dangered off & timbered. It will not be cleaned up.

At Approx. 8:00 PM a roof fall was discovered at 110 block of 3 west' track entry intersection. This was an overcast area where the old 4 north belt passed over the track. The fall was approx. 20 wide and fell 8' above original overcast french bolts. Area was originally bolted with 5,8, & 12 bolts. No injuries.
An unintentional roof fall occurred on the 1st South Main, 25th XC (15+00-15+60) in the #8 Entry. The fall was approximately 16 1/2 feet wide, 8 1/2 feet High and 45 feet long. This area had been dangered out on 8-1-06 due to poor roof conditions.
Area has been mined over and under the rockhouse coal seam. Rock fall is in the no. 6 entry & the no. 7 entry in the intersections. Fall area will be cleaned and the mine roof re-supported. MSHA AND OMSL have been notified and MSHA has looked at the roof fall.

Wildland grass fire caused by a spark of a train burned about 90 acres. The fire was contained around 7:00 pm. Local fire department assisted. All units were released at 10:20 pm (mop-up completed). MSHA was called - message was left - was never called back.

Preliminary investigation indicates a mechanical failure caused material to come in contact with the precalciner housing, igniting the fire. It appears the fire suppression system was triggered but failed to extinguish the fire. A complete investigation report when complete will be forwarded to the local MSHA field office.
Spad 1386 old #3 entry 14' underground in old works fell in intersection. 18' wide x 6' thick and 20' long. Dangered off. Breaker timbers were installed and was using 5' bolts. Fall will not be cleaned up.

A roof fall occurred on #1 unit, #1 entry, at 4 crosscut. The fall measures approximately 18 feet wide, 18 feet long, and 5 feet thick. The area is supported with 4 foot fully grouted resin bolts.
Drill operator said the 992G loader was on fire, when foreman got there the loader operator had gotten off the loader. At 10:45 p.m. the mine office called 911, then in turn called the Logan Fire Department. The Logan Fire Department arrived at 11:15 p.m. - the fire was completely out at 12:30 a.m.
Roof fall was found at the 2530' of #6 entry in the 1st East @ 1:30 pm. Fall was in a 4-way intersection. Fall was 45'L, 17-18'W, 7-8'H. Primary roof support was 5' double lock bolts. Fall will not be cleaned up. Areas have been timbered off & additional roof support has been installed (cribs).
An unintentional roof fall occured along the Main South Belt. The fall occurred due to the fact that the roof strata broke at and below the anchor point. The fall will be cleaned up and area supported.
A roof fall discovered in the 5th left panel # 6 belt entry during an exam. It measured 30' long, 20' wide, and 7' thick. Fall occurred due to laminated shale and coal above the 4' resin roof bolts. It did not impede travel, affect ventilation, no injuries occurred as a result of the fall. The fall occurred in an area developed Jul 05 12,000' from the active section.
On August 3th at approx. 12:30 pm a ground fall occurred in the 1500E stope. The measurements of the fall were approx. 40' in length, 14' wide and 10' deep. The approx. weight of the fall was 300 ton. The stope cut was mined out and waiting for paste backfill at the time of occurrence.
On 2-South #4 hdg - 7 bl fall approx 6'-7' high x 17' w x 40'-60' in length. Cover approx 250' area bolted with 5' resin bolts. The hdg was mined between a full, partial pillared block & solid block of Pittsburgh coal causing roof condition problems in seam. Affected area has been rebolted with 10' & 12' cable bolts & 6' 3/4"" resin bolts & boards. Hdg width narrowed down also.
At about 8:22 a.m. a fire was discovered on belt conveyor 62BC11. The fire was confined to the 62BC11 area and was extinguished by 9:15 a.m. The fire is believed to have been ignited by hot clinker fines contacting the rubber parts of the conveyor. This unexpected release of fines may have been the result of work in the precal system to remove restrictions.
Lost power to whole mine and main hoist for 45 mins. Evacated employees to Main Shop per SOP. Troubleshooted the problem. Found bad 10 amp fuse for the main switch gear circuit board control. Called and left voice mail for Don Ratliff, MSHA Field Supervisor at 3:10 am and called back later when hoist was fixed at 3:30am.
On 8/2/06, unintentional roof fall occurred at the #46 crosscut in #1 entry this was located in the 3rd south, main west return. The area was initially supported with 10' point anchor roof bolts, the fall measured 35' x 20' x 10' high and the roof fall will not be cleaned up.
Roof fall occured in entry #4, section 2-A, 12,000 ft. from section. Area was breakered off. Timbers and jacks were set to establish walkway to seals. Fall was 3 ft. long, 12 ft. wide by 4 ft. high.
After lubricating two of the four hoist ropes it was placed back into service. On the second trip rope slipped from over-lubrication allowing the cable to slip on the hoist drum & damage the canopy. The headframe & loading pocket were inspected & no damage was found. The damaged canopy was removed and exessive lubricant was removed prior to placing hoist back in service.
Inby spad 1486 between 1+2 entry a horse's back fell in track entry 7 breaks outby face, of 002 section. Fall has been cleaned up and bolted. Reported to MSHA 7:55 AM.
A worn air pressure regulator that controls the brake release mechanism, caused the hoist to stop below the collar. The regulator was replaced and air pressure re-adjusted to put the hoist back in service.
A roof fall occurred on the 3W at crosscut 82, entry 6 to 7. The cause of the fall was weak laminated shale. The fall is located in a common aircourse area and is not traveled on a frequent basis.
An unintentional fall of roof occurred in the face area of newly mined #3 entry. The fall pulled approx 4-5 rows of installed roof bolts. The fall was approx 18' x 6 1/2' x 50'.
A roof fall occurred at the 1st North - Main East, Crosscut 30, Entry 5 travelway. The fall measured approximately 53' long x 16' wide x above anchorage. The fall did not result in any damage to equipment or property.

An unintentional fall of roof occurred on #9 belt in the #3 entry at break #64-65. (Spad #9564 was in break #65.) The fall was app. 8-12' thick x 60' long x 11' wide.
A roof fall was discovered during the weekly aircourse examination in the 2 North 5 Mains, 53 crosscut, #6 entry. The fall measures approx. 30' long, 18'wide and 8'thick. The area is supported with 5'fully grouted bolts.
While in the process of bolting the 4020 west ""E"" R-8 heading a large boulder came out of the back. The boulder went beyond the anchor points of the support bolts. It was estimated to weigh between 8 and 10 tons. There were no injuries and minimal damage to the bolter.
A fall of roof 6 ft high, 16 ft wide by 20 ft long occurred on West Mains at the #10 Crosscut between #2 and #3 entry. The area had been supported. Strata was laminated with clay seams. Additional support will be set through the affected area. No equipment or property damage. Fall was called in and reported.
At approx 4:15 pm 18 employees entered the elevator. The elevator traveled approx 6 inches below the top landing and stopped. Schindler was notified, the problem with the elevator corrected at approx 6:00 pm. Employees returned to surface and removed. The main current motor field relay needed adjustment.
Around 11 am, the Main Hoist lost power and would not reset. Electrican immediately called in to troubleshoot and fix problem. The microprocesser went bad and was replaced.
A roof fall was discovered during the weekly aircourse examination in the 2 North 5 Mains, 55 crosscut, #5 entry intersection. The fall measures approx. 30' long, 18'wide and 8'thick. The area is supported with 5'fully grouted resin bolts.
EE was preparing to blacktop the entrance to the shop, when he walked into the path of a payloader that was transporting material from the stock pile. He was run over by the left front tire.
Plywood was covering the manhole inside the kiln. The EE pushed the plywoood forward and walked through the manhole, falling approximately 20'. (The victim died August 21, 2006 as a result of his injuries.)
Employee was working with a hydraulic rotary drill and place hand around the hammer on the drill. His small finger was between the hammer and the drill bit and the operator released the hammer causing it to come down to the drill. The left little finger was crushed. (The entire finger was later amputated).
Employee is currently unavailable for statement. Apparently, employee's foot got pinched between the knuckles of two rail cars as they were being pushed together to be coupled.
Employee was tramming a roof bolting machine towards the face and turning into a right crosscut. He had his right hand outside of the deck area and when the machine turned his hand was caught between frame and the coal rib, mashing his right hand.
Putting pin in foot jack, jack raised crushing rt. hand.
Lease truck driver tried to remove a rock stuck in his tailgate, gate slammed on his finger amuptating 1/4 of ring finger on left hand.
EE and another EE were in processed putting canopy on buggy. As they lifted the canopy over the mounts the canopy slippped & pinched his left index finger between the post & mount.
Was pinning top. Drilled hole in top for pin. While installing bolt, he was spinning bolt to mix glue. While positioning plate against top plate, he caught his glove, pulling finger off of his left hand.
Employee experienced pain in groin while at home, and was diagnosed as having a small inguinal hernia and muscle strain, possibly caused from years of climbing on and off heavy equipment.
This is not an employee. He is a customer, He was dusting his tailgate off because it was loaded with sand. His tailgate shut on his right ring finger.
Employee was stepping off platform which was ground level. Employee lost his balance and slipped and fell back onto platform.
EE strained back while pulling on belt (converyor). Started missing work on 9/1/06. He did not report injury until 9/14/06.
EE fell off ladder while working on plant. EE fractured right wrist.
Employee stepped on the ripper motor cover lying on the bottom, twisting his knee. Went to dr 9/13/06--started losing tim 9/14/06.
Employee was hauling supplies on 001-0 section when he ran over rock in roadway causing bruise to lower back.
Miner was cutting a pipe with a hand grinder.
Employee cleaning material from dozer blade; operator lowered blade.
While pulling together a broken belt, the employee was struck in face when chain broke.
Employee bent over to move miner cable out of haulway. Feeling sharp pain in lower back. Went to Dr 9/1/06 - started losing time 9/9/06.
EE DROVE MANTRIP OUTSIDE WITH CREW FROM SECTION AFTER END OF SHIFT. EE GOT OUT OF MANTRIP, WAS APPROACHED BY ANOTHER EE, AN ALTERCATION TOOK PLACE RESULTING IN EE GETTING PUNCHED IN FACE SEVERAL TIMES. BOTH EMPLOYEES WERE IMMEDIATELY SUSPENDED. IT IS UNKNOWN BY COMPANY IF ANY INJURIES RESULTED FROM THIS INCIDENT OR IF FILING OF THIS REPORT IS REQUIRED UNDER THESE CONDITIONS.
Miner was attempting to deflect material from a moving conveyor belt with a shovel. The shovel was caught by the conveyor causing the miners hand to become stuck betwen the shovel and the frame of the machine. The miner was wearing a wedding ring which caused most of the injury. This consisted of laceration to the finger and avulsion of the skin.
This man has a history of diabetes. Anytime he gets a scratch, he gets an infection. He was replacing a bolt in a transmission. He reached down through the floorboard in a D7 Dozer, when he scratched his left elbow. He didn't tell anyone. He got an infection in it and was put in the hospital and on IV's on the 15th of Sept.
Employee was putting a cover over a shoot when dust got in his eye.
Employee was working on the roof of a shovel when the toe of his boot hit the end of a grip strut causing him to trip. He fell onto the strut with his left knee and his left shoulder struck the shovel house filter housing. This caused contusions to his knee and shoulder.
Stepped in a hole after dismounting truck and turned ankle.
Driving down road at speed during plowing operations, hit a bump, causing his face to strike steering wheel. He sustained bruises - missed 2 days work.
The turn pulley on the conveyor was plugged up with aggregate material. Employee shut the machinery off and tried to get the material loose with a shovel. After no luck he tried again with a screwdriver. The force of the material shifting caused the belt to turn backwards and caught his hand in the pulley.
While operating a rock truck, the right side, front suspension cylinder broke where it was welded to the mounting support, this caused the truck to come to a stop causing EE to be thrown forward. He hit his chest on the steering wheel.
Employee exited loader to dislodge material stuck in grizzly. As he dislodged the material, his middle finger of left hand was injured. Medical treatment included pinning of tip of finger.
The EE stated he was swinging a 8lb. sledge hammer to knock blocks out of a stopping when he missed the block and his right arm hit the sharp edge of the gunnite on the stopping. He received six sutures.
Employee was crossing lo lo and fell to the bottom, resulting in a fracture to his left ankle. Employee started missing 09-01-06.
Foot caught between cab of car and floor
Employee was descending a ladder from the welder platform when the heel of his shoe caught, causing him to fall about 3 feet.
The worker was bracing a rock to prevent it from jumping when it was cut. The force of the cut proved too great for him to stop and the rock came at him off the end of the table. A pallet that he had placed behind himself earlier kept him from getting clear of the rock so it landed on his foot. Part of the rock landed above the steel toe crushing the foot and breaking one bone.
The operator started to empty the bucket inside the truck. While emptying it, a rock came out and entered through the window of the excavator, hitting the operator in the chest & causing an accident.
Picking up a broken 100# bag of aquagel from slide before palletizer, when another bag bumped it, causing bentonite dust to get in his left eye under his safety glasses.
Emp doing a preop inspection of truck and hit lower right leg on piece of timber. Reported on 5 point card, did not seek medical treatment. Wound got infected and employee sought treatment and was admitted to hospital for IV antibiotics.
While back draffing pile, hand slipped off steering wheel causing him to strike elbow on steering knob.
Employee was manually clearing rock from the cavity of a 7' cone crusher when he suffered a muscle strain in his lower back.
Injured was unloading cans. He pulled out standard & can rolled on his hand. The tip of his left hand ring finger was shattered and he has two fractures.
The employee was palletizing 50 lb bags of product when he started to feel soreness in his groin area. However, he did not report the incident until the next day. Groin strain was the diagnois.
Employee was using a cutting torch to heat up a bearing at the J-1 conveyor when the base of the torch exploded, burning a spot on his left arm.
Employee was hanging roof bolter cable and walking when his head contacted a belt ""L"" bracket that was installed in the mine roof. The bracket had been installed for a belt move and the roof was approximately 5 feet high. *Note: Referred to specialist for diagnosis on Sept. 14, 2006.*
EE stated he was using a round steel bar approx 6' long and weighing about 10 lbs to dislodge a piece of slate that was lodged in the raw coal chute going to the #1 heavy media vessel. While the EE was pushing on the bar to break the material loose, he felt pain in his right shoulder.
Employee was walking down entry when he tripped on a tire mark and fell on his left shoulder. The area was very soft from a water event. Employee continued working until 12/12/06 when he had surgery performed on his shoulder and began missing work. Information was received on 12/21/06 relating surgery to this incident.
Three employees were pulling off pulley sheave on crusher. They used pusher bolts but it wouldn't come off. Got wheel puller & two employees tightened wheel puller while the other employee applied heat with a torch. Never showed any signs of movement - just jumped off all at once and edge caught one employee in the forehead at the hairline.
Employee was cleaning out cooler-feeder with an air lance. Lime broke loose and blew in his face.
Employee was removing rails in 5A spur when he strained/sprained his back. Employee was using a slate bar to turn a rail which jerked back when the rail caught against an unseen rail bond.
The mining crew was completing a slip form pour in a vertical shaft. A foam ball was put into the slick line to clean it out, when the foam ball was blown through the slickline the hose whipped and struck EE knocking him into the rib and to the top of the form. As a result the EE sustained a head injury and was transported to the local hosp- ital he has a skull fracture.
Employee was coming down a ladder, slipped and twisted his left ankle.
Employee was doing a routine check on caterpillar 785 rock truck and as he exiting from underneath the truck he hit his left shoulder on a pull hook under the bumper and then hit the side of his head on the bympen. He wearing hand hat.
Employee complained of dizzyness as he tried to get off of excavator and lost his balance. Employee fell 4 to 5 feet to the ground cutting openhisarm and hitting his head and shoulder. Employee was planning on fueling up excavator at time of accident.
After roof bolting, employee was assigned to remove and cover shield electrical connection. At some point, employee felt a foreign object in his right eye.
DID NOT START LOSING TIME UNTIL 4/19/07. While lifting concrete mix, strained right wrist. Has been seeing doctor since time of accident. Had surgery on 4/19/07.
Reported 9/5/06 - Helping load a shot and twisted knee.
Analyst dropped pipette bulb on the floor. When she bent over to pick it up, felt pain in the middle of her back.
Employee was operating shuttle car when a slab of rock fell from the roof striking him on top of his head.
Employee was B-Bonding a stopping on right mains. While applying B-Bond he got some on his arms causing skin irritation.
Electrician received an electrical shock while inspecting a trailing cable for a shuttle car at a splice. Employee complained only of pain in the right shoulder. Employee did not lose consciousness, was not burned, and did not show signs or symptoms of cardiac or respiratory distress. Was diagnosed with a fracture to the right scapula.
On 8/29/06 EE was setting up the survey instrument in #8 entry on #1 unit when a roof rock fell out from between roof bolts. The rock struck him on the back of the hard hat and right shoulder driving him to the ground causing a dislocation of the right hip.
Injured was replacing offside tram motor on a shuttle car. A chain hoist was used to raise the motor. The motor shifted and the chain rolled from the top of car frame to the side of the frame and came across injured's left index finger, smashing it.
Employee's hand was caught in hose material and twisted while attempting to manipulate the hose to start slurry flow.
Employee got off his shuttle car and sat down next to coal rib while waiting for continuous miner to move. When he got up he lost his balance and twisted right knee causing a sprain.
EE was using D8T Cat. Dozer w/Semi-U dozer blade w/approx 3050 total hrs on machine. He failed to stop machine & reverse unit to climb back up slope he'd just dozed the material from. His failure to control machine resulted in pushing through safety barrier & causing machine to become upset from its normal operating position. Received cut to left hand.
EE was cleaning up the material that had spilled around the sand screws discharge collecting box with a shovel and in performing this task he slipped and lost his footing and fell thus cutting his right forearm with a 2.5 cm cut. He wasn't sure what actually cut his arm, he was released to come back to work from the clinic but he decided he needed a day of rest.
Backed up to stockpile, raised bed. When pulled forward felt frame raised up. Was thrown against passenger door. Thus spraining ankle.
Employee was holding a piece of pipe stable on the ground with his foot while a co-worker was hitting the pipe with a sledge hammer. The pipe came loose from under employee's foot & bounced up hitting the top of his foot. Swelling did occur & employee complained of stabbing pain.
Employee was cutting a track pad bolt. Pad broke loose striking employee and making him fall to the ground.
Employee was pulling on cables and strained lower stomach. Injury resulted in a hernia that required surgery. Employee started missing 09-20-06.
While attempting to secure a 1 gallon can with Bungee while standing on operator access, bungee broke and operator lost balance falling & breaking ankles.
Pulled shuttle car up behind miner loading coal. a rock came over sideboards hitting him in nose and right arm.
While pinning coal with his loader in the pit, the loader movement strained his neck.
A piece of metal skirting was loose on the Security Shelter located by the stairs.
Employee smashed tip of middle finger under hydraulic press plate on strapping unit of # 4 machine strapping unit at Stargo Ew Tankhouse.
Employee went under screen deck to stop a leak of conveyor belt and removed shovel by kicking it. Employee's right leg got caught up in roller.
Rosk wedged in bottom of hopper door vibrating Grizzly Feeder. One employee was hitting the rock with sledge hammer and a piece of rock broke off and hit another employee in the eye standing adjacent on catwalk causing damage to right eye. Had PPE off to wipe eyes.
A rock fell off a corner in #3 entry outby face. Employee had moved out of the way of a shuttle car going to miner when a rock came out of the top. Hit him on the shoulder, arm & right foot.
DISMOUNTING EQUIPMENT, LANDED WRONG ON LEFT FOOT/LEG.
Employee was bagging lime and felt pain in left elbow, wrist and two small fingers went numb.
Ee stated that he was pulling jumper cables at the 5-A station for his ram car when he felt pain in his lower back. (This accident was not reported until 9/5/06).
EE was working in a loader bucket when he slipped & strained knee.
Hydraulic cylinder on the Kamasu PC-300 was being replaced. EE was on the lader pushing pin thru. Bucket was holding cylinder up. Clay was packed on the back of the bucket. Small amount of clay broke down tram bucket and fell on his head. The ladder went one way and he the other way. He and other the dirt and his left leg buckled under him.
Employee was lifting a timber and felt pain in left side of groin.
Foreign particle embedded in left eye. Employee's eye became irritated after reboarding his truck after the loader completed loading large rocks. The particle was found the next day and removed by medical provider. Due to continued irritation the employee did not report next scheduled shift.
Employee was operating a shuttle car when he hit a hole causing pain to his back and neck.
Employee working on long wall move piece of rock fell between 2 cribs striking him on neck and shoulder.

Employee was loading a bulk truck when cement leaked from the rubber hose and got in his left eye. This caused some burning and the employee was taken off work by the Doctor.
Employee stated that while dismounting a D10 dozer he slipped and twisted his knee and ankle.

EMPLOYEE WAS RIDING OUT ON MAC 8, WHEN HE WAS BOUNCED UP OUT OF SEAT. WHEN EMPLOYEE LANDED HE LANDED ON THE HAND RAIL STRIKING HIS RT SIDE RIB CAGE.
Dismounting water wagon-missed bottom step, fell to ground, left knee injury.
7I at 19 wall track switch. EE got off motor and stepped on a wet rock and slipped twisting left knee. He continued to work normal duties until lost work days began on Feb 14, 2007.
A haul truck driver fell from a distance of 8 ft while boarding a haul truck. While climbing the ladder on the truck the employee's lunch pail, which he was carrying over his right shoulder, hung up on the housing of the air filter box. Employee lost his balance and fell away from the ladder.
Member had finished loading supplies into bucket of scoop and was backing around corner into #2 entry when he ran over a piece of 6"" Ni-Pak line not in use. End of pipe rose up and entered scoop deck striking member in his left arm causing a fracture.
Located at 104 blk, Main North: 2 EE's were taking an air reading with a smoke tube(@ Main East seals). The smoke tube shattered while inserting it into an aspirator bulb, causing a puncture/laceration to his left 5th finger, that required surgery to repair the flexor tendon. Began missing 8/28/06. Surgery on 8/29.
Employee was installing a new chain on feeder breaker (SL05) using a Port-a-Power tool. The tool cylinder jumped out of place, hitting employee on the shin. One week later, employee received medical attention and injury had become infected, resulting in lost work days.
Employee was pushing the pan when a rock came down the panline 6' long and 5' wide and struck him from behind, pushing him out into the pan.
Running the loader to work, the road ran into a railroad track that was under the road. The operator's head hit the windshield of the loader and injured his head, neck and back. He was not wearing his seat belt.
Employee was moving a shovel bucket tooth. He grasped the tooth with both hands and when he moved it he felt pain in his left bicep. He was taken off of work starting 10/11/06 due to surgery.
The employee stated he was pulling pipe and felt a pain in his lower back and left rib. The employee continued to work until 8/29/06. He then went to the doctor and has been previously diagnosed with lumbar bulging disc and osteoarthritis in the lower back.
Employee was roof bolting and while adding a drill steel, he hit the up lever and caught his right thumb between the steels.
Employee was climbing off of equipment & fell off ladder landing to the ground. He complained of Lower hip & back pain.
Injured was spraying wall with sealant and the hose came loose and he inhaled fumes.
EMPLOYEE HIT BUMP OR ROCK IN ROADWAY CAUSING HIM TO BE BOUNCED UP OFF OF SEAT. WHEN HE LANDED BACK IN THE SEAT HE LANDED ON HIS LOWER BACK.
The employee stated that he was opening the main drain valve on the sprayer line and was sprayed with 280 degree liquor. A smaller drain valve on the side of the line was already open and the employee did not know.
When leaving pit, employee thought he had hit button for cushion hitch, but scraper began to bounce uncontrollably. He hit his head on the left side window, twisted his back and had the wind knocked out of him. Back discomfort caused him to miss days at work. Seatbelt was being worn.
: MW#4 - Employee was bolting on roof fall. Rock fell from between bolts, hitting the ATRS and sliding off. Struck employee's right lower leg. Rock was 29"" x 31"" x 1"".
Member was welding on a bit lug on a cont mining machine & the arc from the welder burnt his eyes. Employee did not notify anyone of this until 8/29/06.
EE stated that he was changing conveyor belt structure out when he felt pain in his lower back. Ee went to the E.R.Dr where it was determined that he had suffered a lower back strain.
Employee was taking water line apart, took coupler off and water line sprung upward hitting him in the face.
Employee states he was jacking a Kennedy panel up when some dust come down and went into his left eye. They washed his eye out and he worked his next shift then went to the doctor on 8/27/2006.
Employee was using a bar to open the bottom door on an incoming ore railroad car. When the door latch mechanism suddenly released, employee felt pain in his back and left arm. Doctor's diagnosis was lumbar strain and muscle strain.
EE was attempting to loosen a bolt for the drive mechanism on 34BC08 and strained his right shoulder while pulling on a wrench.
Employee bent over to pick up hand grinder, felt back pain.
EE was injured on 7/27/06. He returned to work on 8/9/06 on regular duty. He stated that his shoulder started hurting again. On 8/25/06.
Employee stepped off ladder onto a block and twisted knee. Started missing work 8/28/06.
Bolter operator ws exiting the canopy compartment, when his knee pad became lodged under canopy deck, causing his knee to twist.
Climbing access ladder to conveyor. Employee did not seek medical treatment until 8/29/06, restrictions were issued that day.
While assisting in the repairs of a 30"" feed belt, ee cut the knuckle of his left index finger after coming into contact with a sharp v-belt pulley. He at first refused medical treatment, but was later convinced he should go. He received 4 stitches.
Pulling on chain of a chain hoist to hoist object, employee hit arm on door jam.
Employee twisted his body while lifting a stone causing a rib fracture.
Was moving a section of track 30' long when moved it slipped rail fell back striking employee on top of right foot.
While positioning miner a rock fell from between roof bolts measuring approximately 4'x3'x0-3"" and struck him on the back and hips.
Removing door from crusher - hit in chest with door handle.
While reassembling the jaw crusher, EE pinched his finger between a chain and a bearing. He received a laceration that required sutures.
Employee was attempting to break off a roof bolt which had bent during installation. He had used a hacksaw to partially saw through the bolt and was rocking the bolt back and forth to break it off. He gave one extremely hard push, the bolt broke off, he lost his balance falling onto the roof bolt plate causing a laceration on his right forearm.
Employee fell through grating cutting his L Leg.
Employee was bending a 5' pin when he felt his shoulder pop.
EE was attempting to roll rock out of track when it rolled back the wrong way pinning his leg betwene motor and rock, He started having trouble with his knee and went to dr and dr profomed microsopic surgery on his knee 4/9/07 causing him to be off work accident occured 100' outby spad #572 2nd northwest mains.
The miner was digging out around the cone discharge when he felt his back pop out. He went to the emergency room, and then to a clinic for care. He currently released to restricted duty, but is not working.
Ted was lifting a chain guard and felt pain in his back. As per Dr. Thompson's diagnosis: an MRI showed degenerative disc diesease with possible impingement of the left side nerve root fibers at IAT5. Ted became reportabel as of 9/19/06, because of the Dr's diagnosis, this is classifed as an illness.
Operator was cleaning accumulated material near the feeder pan of the processing plant when he pinched his right index finger. The tip of the finger bone was broken and a laceration required stitches to close.
EE was helping load a tail roller. As he lifted a 6"" sheive wheel to hang on bolt to pull the roller with, he developed pain in his low back. He was taken to the Williamson A.R.H. hospital for treatment.
Employee was moving a suction hose & pump in the pit area with a forklift. Hit a low spot in the pit area and the forklift leaned over onto two wheels. He jumped from forklift spraining his ankle.
Employee and a mechanic were in the cab of the loader trouble shooting an electrical problem. They were traveling down the road and hit a bump. The employee hit his coccyx bone on the cup holder in the cab. It bruised and fractured his coccyx.
After dismounting the #386 dozer, employee took a step and went to the ground, rolling over once. Employee got up the second time and went down on the ground on his left side. Employee stated his right knee had given out and was hurting. He was seen by his physician and told to follow up with a specialist in 2 weeks. Ongoing.
Employee slipped getting off equipment and strained back and knee.
Employee was in the process of drilling a hole in the roof during roof bolting procedures. Ee placed his hand against the boom of the bolting machine to balance himself preparing to remove the drill steel from drilled hole. Suddenly a piece of draw rock fell measuring approx 1.5'x2'x1""-3"" thick striking him on side of index finger fracturing & lacerating his finger.
Employee felt a twinge of discomfort in his back at work on 8/24/06. On 8/28/06 he had severe back pain and left work. His back had also hurt over the weekend while doing housework. Employee was unsure whether injury was work related. Doctor was also unsure. On 9/6/06 doctor determined injury to be work related. Employee did not report an injury until 8/28/06.
Employee was cleaning out chute. He ""hit the chute with a bar and smashed his right index finger between the bar and a beam"". Avulsion to right index finger.
Employee cut a tree. As the tree was falling, it hit another tree breaking a limb out of the cut tree. When the limb hit the ground, it came back and hit him in the arm.
Employee was removing track shim when the jack cylinder retracted smashing his fingers
Two EEs were moving a tote container with approximately 20-50 lbs of product when EE experienced very sharp pain in lower back. EE's were using push-pull method when injured EE twistd to see the direction and area into which they were heading. When EE twisted it appeared that his back went out, causing very sharp pain to lower back.
EE was hanging power cable when he felt pain in upper right arm.
While lifting a heavily loaded garbage can, the EE's right shoulder popped resulting in extreme pain for the EE. He was diagnosed with a strained joint and taken off work until his next work rotation, missing his next shift, pm of 8/24. He reported to work on his next rotation, 8/27 and hurt too much to continue working and went home, called requesting to see a DR. ONGOING.
Employee was getting off of a piece of equipment and simply twisted his left knee and felt it pop. The incident occurred on 8/24/06 but he did not seek medical attention until 8/29/06. At this time he was diagnosed with a tendon strain and was placed on light duty.
Employee had laid a timber aside, when it rolled off striking him on the leg and foot.
Employee was hanging a block curtain. Stepped in a hole, twisting left knee. Employee did not start losing time until 10/30/06.
EE was clearing trees and stumps preparing for the next advancing pit. He was backing up and hit a stump, dozer came to a sudden stop.
Employee was clearing rock from feed box when rock fell onto right hand injuring right thumb
Employee using air sander to clean blade on grader. Sanding disk came apart, piece struck employee in the stomach.
While removing a piece of rebar from screen chute as he was lifting and turning the rebar caught screen stopping his motion causing him to feel a pain in his back.
While cleaning rock off of the miner, a piece of rock fell off of the miner and struck the employee on the right foot. The rock was approximately 2' x 1 1/2' x 8"" thick.
Was on catwalk, rotated to face the opposite way & twisted knee. Knee popped.
A piece of rock fell striking victim on head. Victim went to hospital received x-rays. Doctors stated victim had bruised muscles in neck. Victim returned to work on regular duty on 8/25/06.
Miner was setting pump discharge pipe wehn he slipped and fell.
Employee was blocking a shield in 8h and strained rt shoulder.
Truck driver fell off from the back step approximately 3 ft. while tarping trailer and broke his right leg.
The employee stated that he was opening a drain valve and caustic ran down in his boot. This injury was minor then it worsened and now is a LTA.
EE was making an examination of the B-5 belt and stepped on a piece of slate. He twisted his right knee causing a strain to the right knee. He continued to work on his regular job until he went to a physician and was taken off work on September 12, 2006.
Employee was helping pull equipment cables during a belt move, when he felt a sharp pain in his back. He completed his shift and missed 4 days work.
Located on the Tailgate #2 section. While disassembling belt structure, employee was struck on the left calf, just above the boot line, by a belt stand. Employee received a laceration that required 13 sutures. Began missing work on 8-24-06.
This questionable incident was to have occurred while this employee was shoveling a coal spill at the LW tail. He claims to had his foot slip off the shovel hyperextending his knee. Cause of injury (if occurred) was awareness/attitude. Compliance not a factor, protective equipment (knee support) a factor, miner attitude a factor, mining equip/systems not a factor.
Employee cut a tree. As the tree fell a dead limb became dislodged and fell striking him in the left shoulder. Limb was not seen during preshift or puring observation of tree before cutting.
HE WAS MOVING UP CABLE , WHEN HE STRUCK HIS ELBOW ON CANOPY CAUSING PAIN. ** STARTED MISSING ON05/15/07**
Employee was exiting the right side of a dozer when he lost his balance and fell backwards onto the ground. The employee stated he hit on his left buttocks and right shoulder. He got up off the ground under his own power.
Employee strained abdomen area as he was lifting cribs blocks. He did not start missing work until 8/30/06 due to hernis surgery
Employee running 50 lb bag dust went to move one bag, had pain in left shoulder.
Member was driving a diesel tractor, the tractor started to bounce, as member hit brake, causing member to hurt right elbow.
Operator erroneously handled the change of a roller. He did not use the appropriate tool for the link of the roller. When introducing the hands directly to the roller, roller was expelled & caused the operator an impact to the face. Took him to the hospital and they diagnosed a minor wound to his nose.
Employee was operating a roller during construction of a mine haul road. He drove up an incline to turn around ans as he was backing up lost control of the roller, drove over the edge of the road under construciton and down approximately 150 of steep hillside., coming to rest upright at the bottom of the slope.
Employee was installing bucket elevator's equipment when the bar he was holding slipped causing the bucket to jerk his arm & shoulder. *Note: Employee was released to return to work with restr ictions. He never returned to work and therefore was terminated.
Repairing piece of equipment & a silver of grinding debris flew in his eye.
Employee was chipping slag & a piece of metal got under his safety glasses and into his right eye.
Employee was climbing off 980 coal loader when he lost his footing and fell to the ground and injured his back.
Lifting of a welder into welding truck causing a lower back strain.
While Building a brattice in the x cut between #7 & #8 entry EE was working 18"" away from a check curtain. A 488 Long airdex scoop after stopping and sounding and his bell started through the curtain and hit him.
A piece of rock fell out from between the straps and struck him on the head and above the right eye causing a laceration requiring 5 sutures.
Employee slipped while walking down entry and fell on left leg.
While standing on step ladder fueling a tank. Step broke causing ee to land on his feet (hard) on the ground, injuring knee.
Member was crossing over a rib roll when a rock laying on the rib roll slid and struck his left knee. First day away from work 8/24/06.
Employee was replacing steering part on a 777D rock truck. He was using a hammer to drive new ball joints into position. During this time he had a pain in his right hand and lower arm.
Empoyee was moving shuttle car tires. As he was attempting to move one of them, it started to roll away. When it started to roll toward the employee, the employee tried to get out of it's way and divert it at the same time. He experienced a burning sensation and felt a ""pop"" in the right groin area.
The injured emp. was struck ont he lt. thigh area by a comalong when a 5/8"" bolt broke that secured a supprot bracket of the 7d storage unit.
Miner operator was loading coal into shuttle car. When the car moved forward, the tire rolled into miner operators foot. (Remote miner) This resulted in bruising around the ankle and side of foot.
Employee was cutting metal with oxygen and acetylene torch when a piece of hot slag struck him on his right elbow, causing a small burn. This injury was not reported to his employer until 8-24-06 & ee worked thru 8-25-06 at which time he sought medical attention. He was admitted to a local hospital & diagnosed with an infected bursa sack that required surgery to drain.
Employee was climbing ladder to operator's cab of a Cat 60 ton haul truck when he felt a pain in his lower back.
While cooling a bearing race with propane, another employee was heating the bearing boss with an open flame, approx 30 ft away. Propane fumes ignited. Employee was burned by flash on arms.
The injured and another EE were setting 100-ton roof support jack. The injured struck his head against a cable bolt head, which was protruding from the mine roof, causing a strain type injury to his neck.
Employee was plug and feathering a large rock when a portion of it fell on his left leg and broke it.

Employee was removing a hydraulic hose from an excavator. After hose was removed, the employee's hand continued to travel because of the force exerted to remove the hose, hitting the excavator boom's pin.
Employees were crushing as they normally do. Injured saw a block out of place under the hammer leg which needed to be pushed back. While he was doing that, a rock fell from the crusher hitting him in the head.
Employee slipped on a significant amount of water that was on the kitchen floor, and fell. When the employee fell, she hit her head on the floor, causing her hair clip to cut her scalp. Employee became disoriented, dizzy, and had a headache. After a period of time had passed, she vomitted.
Working on primary crusher he dropped his wire brush onto D4 conveyor. He climbed down to get the wire brush. As he climbed out and off the conveyor he went to jump over the railing and his right knee locked up & he could not bend it.
Employee was descending a ladder, lost his balance and fell to the floor. He extended his right arm to cushion the fall and fractured his arm.He was evaluated by a physician and was taken off of work.
Worker pulling on come-a-longs, prying with pry bars, pulling chainfalls, unsure what work he was performing when accident occurred.
When mounting the 007 LHD employee's right foot slipped, causing him to catch himself with his left arm. After which he felt a pull in his left shoulder.
WHILE REPLACING A BEARING WITH A HAMMER, A SHARP PIECE OF BEARING SEPERATED AND IMBEDDED ITSELF IN THE MINERS MIDDLE FINGER ON HIS LEFT HAND.
Employee was a passenger on SV-11. The vehicle engine stopped. Before the brakes set, the vehicle hit the rib causing the engine cover to strike the employee's legs, breaking both legs.
While descending loader using 3 point contact, facing steps employee tried to place left foot on second rung of four when lost footing and fell to ground four feet. She hit the pavement below on back, left hip and back of head. Employee said was ok and refused medical attention at time of incident. 8/21/2006 she was determined to have sustained a concusion.
The injured is a roofbolter helper when stepping over the cable he stepped on uneven slick place of mine floor.
The employee was injured while tramming a battery operated scoop. While traming across the section the operator ran over some debris in the roadway causing sudden movement. Shortly afterwards the operator started experiencing lower back pain which continued to worsen over several months. After seeking medical attention on 4/17/07 He was taken off work due to lower back strain.
Employee was chipping a recent weld when some hot slag was chipped onto his left elbow. The employee waited two weeks to seek medical attention when the elbow became red and swollen. He did not indicate injury was definitely he related until 10/09/06 ( the return to work date).
EE was replacing the rear suspension cylinder on a CAT 793 truck. As he was using a sledge hammer to drive the top strut pin from its bore he felt a pain in his right wrist. He did not report the incident at that time. On 9/22/06, as he was loosening/tightening the wear plate retaining bolts on a 24M motor grader, using a 3/4"" impact wrench, he again experienced pain in wrist.
emp. was attempting to release hand brake on railcar. He grabbed a 3-foot bar and placed to release brake. emp, felt pain in left shoulder. Emp, went to doctor six days later. doctor said no findings, in 04/07 he asked for 2nd opinion. worker's comp and attorney have been disbuting claim. finally a specialist elected to have surgery. 1st day of work missed was 12/3/07
Operator said that the truck stalled as he was leaving loading area and he was unable to maintain control of vehicle. Vehicle overturned, operator put hand outside of window and arm was caught under mirror and mirror bracket that were crushed during rollover causing hand to be de-gloved and to break arm in couple of locations.
Employee was using a box blade knife to cut a piece of conveyor belting during a repair to the conveyor belt. The knife broke causing him to cut his right thumb. Required surgery to repair a severed tendon. During mandatory drug testing due to the accident, he tested positive for Meth and Marijuana and was subsequently terminated.
The injured employee slipped off the deck of a D-9 Dozer onto the tracks, landing on his tailbone which caused a muscle spasm in his lower back
Employee was putting auger back together, while picking up skid with boom truck, the skid slipped & pinched fingers on left hand & broke left ring finger.
While moving a 9 7/8"" drill bit on the drill deck the employee over-extended his body which resulted in back strain.
Walking, stepped on lump of coal, fell and twisted right knee.
Employee was attempting to pull drill steel from hole when a piece of draw rock fell striking the canopy and hitting him in head.
cutting stone and stone fell and hit employee's right hand - causing a contusion
Loader operator was going forward not knowing a car was behind him. Operator went into reverse, car driver tried to reverse, Operator accidently hit his car. Driver opened his door, fell out of car onto pavement hitting his head.
Employee was using jackhammer when vibrations caused loose cracked rock to fall. He tried to get out of the way, but was too late. One large rock fell and hit him on the elbow, causing swelling and a small cut. EE is awaiting his surgery date for a pinched nerve.
Employee had installed outside bolt and moved drill head to put up inside bolt. A piece of rock approximately 1 inch thick x 4 foot wide x 5 foot long fell and struck in on the back of his right ankle due to employee working in a kneeling position.
EE was doing standup, he slipped on stone while walking stone onto pallet. When he slipped, the stone came down on his back, it broke and a piece hit him on right lower leg.
EE was lighting a burn pile (broken wood pallets) and got to close to the frame. He burned his right arm & right side of face.
Employee was loading rock truck in pit. When he was filling his bucket, he hit a large rock in the spool and injured his back.
Employee was helping to remove excess rock from the rolling table. The rock he was moving got stuck between rollers. As he was moving the rock, the strap became loose and the rock fell and struck him on the top of his left foot. Upon doctor's visit foot was found to be fractured.
Ee was helping unload a new vertical air compressor that had just been delivered. The compressor had been placed in a loader bucket from the delivery truck and was being unloaded into the shop. He strained his back while trying to get the compressor out of the loader bucket.
Employee replacing skirting on belt. While cutting out notch for bolt holes, the utility knife he was using slipped and he cut himself on his left wrist.
Employee was repairing a Mack 600 fuel truck. When he stepped off the truck he sprained his ankle when he stepped on a rock.
EE strained his lower back after he and a co-worker lifted a freon recovery unit onto a truck. This task should have been completed with a forklift. Employee initially went to the doctor on 11/14/06; however, restricted duty was prescribed on 12/15/06.
Injured was installing roof bolt who bolt hit crack in roof causing bolt to strike hand.
Employee stated that while operating the 95 Kamatsu end-loader, he backed over the berm and turned it over on it's side.
Employee was making a belt move up. The scoop operator was pulling the tailpiece up. One of the chains slipped causing the tailpiece to go side-ways catching employee between the rib and the tailpiece.
Worker was welding on the cone crusher when solvent fumes from cleaning were ignited. Worker was burned by the escaping flame. Worker received 1st and 2nd degree burns to his left arm/right chest/right shoulder/ear.
Employee was climbing out of truck. She grabbed the handle, but slipped off the foot hold and fell. She struck her lower back on the top step of the ladder. She felt fine at first, but later experienced numbness & tingling in her legs. She requested & received emergency transport to the hospital. She is currently not working as we have no ""restricted duties"" for her to perform.
EMPLOYEE HIT ELBOW ON FIRE SUPPRESSION BRACKET, WHEN HE WAS DRIVING MANTRIP.
After glancing off the second truck he collided with the rear of a tractor trailer owned by contractor that was parked immediately in front of second truck. The back of the trailer was knocked over the hill turning the tractor sideways the 350 truck continued on striking the tractor in the sleeper section& rear tandems. Truck pushed tractor on R side pushed down hill 30 feet.
The injured tripped on the rough bottom in the #1 recovery entry of 23 Butt Longwall section. The injured struck his face and left knee on the bottom. He suffered an abrasion to his nose and a meniscus tear of the left knee.
Employee was putting a piece of top belt structure in scoop bucket. He told scoop operator to let bucket down; when bucket came down it landed on employee's left foot resulting in a fracture.
Member was picking up scrap metal raised up hitting head on a pipe hurting neck & back. First day off work was 8/21/06.
Employee jarred his back while driving a golf cart. He continued to work until August 28, 2006. His first missed scheduled work day was Aug. 29. He is scheduled for an MRI on 9/7/06.
Stepping down off of a haul truck and pulled a muscle in right leg.
EE was getting down from his loader and began to fall, he stuck his foot out to try and catch himself and struck his foot on a piece of metal. He stubbed his foot at the toe.
Belt man was traveling #2 haulway, lost his footing on brake pedal and slid in to the rib brushing chest area; still finished his shift.
EE alleges to have injured his back while going thru a mandoor. This is a re-occurrence of a pre-existing injury.
Employee was standing on a 5 gallon pail that he was using as a platform to do some welding work. He fell off causing bruising and contusion to his lower back and right knee.
