Accident type, without injuries

While making weekly exam, it was observed that the brakes on the access cage to not be working properly. An evacuation was initiated at 11:20am. After solving problem, cage put back in service at approximately 12:01pm.

At approximately 4:45 pm the roof adjacent to the last shield on the face collapsed. The fall resulted in a tailgate blockage that lasted for 2 passes.

An unintentional roof fall occurred on the 2nd main west at the 13XC (7+80) in the #7 entry. The fall was approx. 30'long x 18'wide x 12'high. The fall was primarily in the intersection on the entry which is a 4-way intersection.

Around 2:30pm, an environmental specialist noticed smoke in a remote location and went to the area to investigate. Upon arrival, he noticed that there was a fire in a cut over timber area. No one was working in this area. About fifty acres of cut over timber burned.

a roof fall was discovered int he no.3 entry of 1 north (survey station 162) about 5 miles outby the active sectior. The fall measured approx. 40 ft long x 20 ft wide x 5 ft high. There were no injuries or equipment involved. Ventilation has no affected. The fall will not be cleaned up.

The area of the fall between #3 & #4 entry in the #4 belt outby had several cribs built. The roof became wet and fell between the cribs. The fall is near spad #407. More cribs were built and 10' rope bolts installed before face work began.

The East Finley Elevator was down for more than 30 minutes due to a bent brake activation linkage rod. There was no entrapment.

During the mine examination a rock fall was found. the fall accured int he #6 entry on the 4 mains, one break outby spad #3304. The fall measured 4 foot thick, 20 feet long and 20 feet wide.

6/27/06 Day shift took a 6' round in the 35E-7700 ramp & fill stope at end of shift. Night shift was mucking, noticed back was trickling & pulled out. 6/28/06 Stillwater rock mechanics came down to have a look & the fall was reported to MSHA on 6/28/06. We are currently shot creteing & bolting the area. Over break dimensions are 10'x 10'x 18', 6' of which include blasted round.

An unintentional roof fall was discovered at the #10 block, #6 track entry, 2 breaks inby spad station 3949. The fall measured 10'H x 19'W x 40'L. There were no injuries or equipment damage.

Rock fall occurred between #3 & #4 entries of 1 East Mains at break 25. Fall measured approx. 50' long x 18'-20' wide x 5' thick.

East 1, 17 block, 3 entry - 4-way intersection. Support: 6'3/4""resin bolts, screen, pans, 12'cable truss bolts. Fall dangered off, additional roof support installed around fall. 18'wide x 20'long x 8'high. Clean-up plan developed and posted at fall site.

A roof fall occurred on the 2 Left 4 West panel in entry number 4 between crosscuts 40 and 41. This was the 3A belt entry and it was supported with 5 foot double lock bolts and timbers. The area was wet which allowed the timbers to sink into the fire clay bottom allowing the fall to occur. The roof in this area is laminated sandy shale commonly referred to as ""stack rock"".

A roof fall occured in the south sub main, #11 Entry, #36 crosscut, 2690 station. The fall was approx 25' long, 25' wide and 8' tall. The area was bolted with 6' + 8' point anchor bolts. The fall didn't affect ventilation, travel, cause injury or damage equipment.

A fall of roof occurred at the #6 BK intersection off the E-1 track entery just outby #15 seal. Fall was in the intersection and was approx. 16' X 16' X 6' high. Area was cleaned up and supported.

The mine experienced a power outage to the Contrary Elevator during which time 3 employees were on the elevator approximately 85 minutes. Power was restored and the employees were brought to the surface without injury.

Roof fall in no. 1 entry 70 ft inby survey 2012. The fall was approx. 19 ft wide, 4 to 5 ft high and 30 ft long. The fall occurred just inby the break which had one cut taken out of it and fell all the way to face.

Roof fall in N36 falling on berm and sliding out into NW-8 roadway partialy blocking access. Area was inspected and cleaned up of loose material. Area adjacent to this had been glued, bolted and strapped.

An unintentional roof fall above anchorage occurred in #3 entry BK#16 Spad #87. Personnell or equipment were not involved. Fall did not block air course or escapeway. Nor does it block the passage of employees. Fall has been breakered off. Fall was approx 6' thick, 20'wide 40' long

A roof fall occurred in the 3 East Panel in #9 entry in the intersection of cross-cut #65 measuring 19ft. x 19ft. x 5ft. The area was supported with 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause.

There was a ground fall at the 3920 intersection. A slab approximately 3'x15' blocked the main ramp at the 3920. An electrical cable fell over the bucket of a loader (mucker). All personnel on Rodeo side from the 4100 to the 3620 and on Meikle side 8040 haulage to 1600 were evacuated.

A roof fall occurred in the 3 East Panel in #10 entry in the pillarline between cross-cut #63 and cross-cut #64 measuring 30ft. x 19ft. x 5ft. The area was supported with 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause.

An unplanned roof fall occurred on #2 section, entry #4 at spad 2135. The fall was approx. 2900' from the working face. The fall measured 30'long, 20'wide and 3'-6'thick. The fall was called in on Saturday, 6/24/06 & left a message on an answering machine.

A fall of supported roof has occurred outby the active sections along the #3 conveyor belt in the #5 entry 1 break inby spad station #1083. The fall measured approximately 20' wide x 8' high x 40' long. No injuries resulted from this fall.

Miner was backing out of lift when shuttle car went down behind miner, allowing rock to fall on miner and causing miner to get stuck at 4:30pm. Before we could get s/car running and miner moved 7' of rock fell on the miner, causing us to use retriever on 003 section.

An unintentional roof fall occurred in the 4I section in a previously mined entry. The fall caused 0 injuries and 0 damage to equipment. The fall was approx 20'x12'x12.

At approx. 1:20 pm on the miner unit 003-0 the continuous miner cut into an unchartered gas well there was an inundation of 7% CH4, 17.3 02 and 105 PPM Co.

Roof fall on the Main South A beltline along the haul road or travelway. No equipment lost. We will not clean it up. Section was bolted with 5 ft fully grouted glue. Fall measured 50 ft long, 18 ft wide and 8 ft thick. Fall did not affect ventilation or passage of men.

On June 21,2006 an unplanned roof fall occured on the #7 belt. The fall is approximately 4-6' thick measuring approx. 25' long and 25' wide. The area will be cleaned up and bolted.

The #2 coal mill system rupture disc was ruptured due to an overpressure inside the coal mill, duct work, or coal cyclone. This was caused by an unplanned ingnition of coal dust. There were no personal injuries or property damage associated with this incident.

HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 06/19/2006 at 03:00 pm, the Cowling Portal Elevator went down due to an electrical problem. David Rinehart, Electrical Engineer worked on the issue. The elevator was repaired and running on Monday, 06/19/2006 at 03:45 pm. MSHA was on-site. Hoisting incident was called in to MSHA at 03:40 pm.

An unintentional roof fall was discovered behind the #11 scal that was being constructed. The fall occurred over the weekend while no one was present. Discovered on 6/19/06 by foreman at spad #5050. The fall was estimated to be 10' thick X 20'wide X 40' long. Slick and slide rock were present in the fall. MSHA inspector investigated the fall while examining the sealed area.

At 2:00 pm a fall was discovered in the #1 entry, outby at spad #17319. The fall measured 25' x 18' x 4' thick. It is being cleaned up and additional support will be added as needed. Both MSHA and OMSL were notified immediately.

The engine on a rock truck caught on fire. It looked like a hose blew spewing fuel or oil onto hot engine.

Oil and grease ignited inside the hulk. After 10 min. the contractors needed assistance on putting out the fire. An EE arrived on the scene within a few mins. and summoned additional on-site operator personnel and equipment. The water truck arrived within a few minutes and doused the hulk to extinguish the fire. No property was materially damaged and no injuries.

#2 Unit had a rockfall inby the dumping point in the #5 room at 2+85 to 3+20. The face was 120 feet inby the crosscut. There was a slip. The fall was in the pillar line.

A roof fall occurred approximately 100 feet inby the mine opening in the #1 entry. The fall was approximately 25 feet long x 2 feet thick x 18 feet wide. There were no injuries or property damage.

An unintentional roof fall occurred on the 1st sub-south return in #2 entry at #47 crosscut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 40' long x 20' wide x 10' high and the roof fall will not be cleaned up.

#5 Hoist outage June 17th from 7:05 am to 7:45 am. Fuses blown on the hoist power supply due to a voltage spike from the utilities supplier incoming power. fuses replace and hoist reset. 3 other hoists available to hoist underground miners out of the mine.

The fall occurred in the 17 East X 33 South intersection. The size was aprox. 50' X 75' x !2' deep. This was a known problem area and was blocked off Friday night prior to the fall. The fall occurred on an off shift and was reported as soon as it was determined to be reportable. The fall did not impede passage, effect ventilation, cause personel injury or damage equipment.

A malfunction occurred to the elevator due to a grounded wiring to the door switch on the bottom of the elevator. No one was in the elevator at this time.

The enlow elevator was down for more than 30 minuets due to the door operator motor going bad. There was no entrapment.

Roof fall at break 11 in #6 entry in 1st north panel. Fall was 19' long, 19' wide and 5.5' thick. No equipment was involved in this fall. The area was cribbed and dangered off.

Roof fall at 1780' mark in xcut between road & belt in 1A east entries. Fall is 55' long x 5-6' high x 18' wide. Intersection on roadway was previously cable bolted when area was developed. Roadway & belt line has been cribbed off. Primary roof support was 5' double lock bolts.

The slope car was being used to transport a longwall load center underground. The heavy load center caused the slope car to derail. Air bags were used to re-rail the car and load was distributed differently.

Computer went down, evacuation started at 8:25 a.m. and completed at 8:37 a.m. the computer was rebooted at 8:45 a.m. and miners returned underground at 8:45 a.m.

The Cedar Creek service elevator was out of service from approx 4:15 am to 8:00 am due to problems with power supply to drive.

At app. 8:45 am longwall supervisor observed a roof fall in the 8 left panel at 29 x-cut track to intake. The fall was 7' high, 40' long and fell rib to rib 16.6' wide. The area was cribbed on both sides and dangered off. MSHA was notified at 8:55 am.

A roof fall occured in the 1 main south, #9 entry, in the pillar line between crosscut #31 and #32. The fall measured approximately 35'X19'X6'. The area was supported with 6' tension rebar bolts. An undetected slip was determined to be the cause of the fall.

Hoist operator notified management of a probable failure of the East production hoist motor. Communication was initiated to begin immediate evacuation of all underground 3rd shift employees. The evacuation coincided with the normal shift change time. Dayshift employees were kept on the surface until set-up done to the prodution hoist for mantrip for second escapeway.

#2 unit had a rockfall inby the dumping point in the intersection of #8 room at 2+75. The face was 15 feet inby the crosscut. There was a slip.

Turning a right hand crosscut in the #7 entry, after loading (8) eight shuttle cars with coal an ignition occurred 71' 11"" inby SS#476. No injury was reported no ventilation was disturbed and no equipment was damaged.

An unplanned roof fall occured in #130 mine in the #8 entry near spad #5273. The fall measured 50' in length, 20' in width, and 6' to 10' in height.

An ignition occurred at approx. 9:15 am in the #1 block in the #2 heading on the #1 section (002). This was the first left hand cut in block. No equipment damage or personal injury involved. No violations were issued by MSHA or DMME.

On 6/15/2006, an unintentional roof fall occurred at the #27 crosscut in #1 entry located in the #2 working section. The area was initially supported with 10' point anchor roof bolts. The fall measured 25' x 20' x 15'.

Methane ignition occurred on the 001-0 MMU in the No. 4 entry, the flames were extinguished using the water sprays on the head of the continuous miner.

A fall was discovered on 6/15/2006 in the #2 entry 2400' outby the active working section. The area was mined in Janurary 2006. 48"" resign bolts were being used with 6' torque tension bolts. Fall will not be cleaned up. All approaches are being timbered or cribbed. No passage of men was obstructed. No injuries reported. Fall was 23' long by 20'wide by7' high.

A roof fall occurred at the 1st North Main East #1 Entry, #81 Crosscut (3-way intersection). The fall measured approximately 56' long x 36' wide x above anchorage. The fall did not result in any damage to equipment or property.

A roof fall occureed in the 2nd right return in # 1 entry at # 17 crosscut. The roof fall was discovered at around 4:00 PM on June 14t 2006. District was notified at 4:12 pm. The roof fall measured approx. 18 feet wide 8' to 10' feet high and approx 55 feet long. The 2nd right section was evacuated and plan developed to clean and bolt the fall area.

South 1, 69 block 9 entry, 4-way intersection. 40 ft long x 20 ft wide x 8 ft high. Support: 6 ft 3/4 inch resin bolts, pans, brow straps. Fall dangered off, additional roof support installed around fall.

Unplanned rock fall 23 breaks from the face. No injuries, no air blockage. Occurred in the old works.

Rock fall on the 90 belt across from spad # 37017 in the #4 entry over to the #2 entry. The fall is about 150 foot long, 20' wide and 8 foot thick. The area was bolted with 5 foot torque tension bolts. We will clean up one entry of the fall and put steel canopy in it. All areas of the fall is dangered off. The fall is about 3000 foot from the face.

Gate swithch came open as a result of salt acc- umulation loosened on platform because of platform cleaning. Miners were evacuated at 8:00 a.m. evacuation was completed at 8:25 a.m. the platform was cleaned, the hoist was tested and the miners returned underground at 9:00 a.m.

Working place #6 entry cut back into 10 rm causing a three way intersection. The breakthru at 10 rm fell. 4 ft high, 17 ft wide and 20 ft long.

Service hoist brakes had no air pressure getting past the main panel. This locked the brakes in the closed position. Mine evacuation was started at 4:00 am through the production shaft and the Newark field office was notified by voice message. The air dryer was repaired and the hoist was functional. Miners were sent back in the mine at 5:15 am.

During the mine examination a rock fall was found. The fall occurred in the #1 entry & #2 entry and adjoining crosscut, located in the 4th break inby the portal. The fall was approx 5-6 ft thick, 18-20 ft wide.

A roof fall occured on the 003 section belt at # 13 crosscut inby spad # 21520. The fall was approx. 8' high X 20' wide X 18' long. The area had been bolted with 5' High tech bolts.

Fire boss found roof fall at 5:15 am on 6/12/06) on working section of #1 North 1 blk inby station #2514 in #5 entry 2 blks inby feeder dump point. On 6/13/06 at 6:00 am fall had continued left to #4 & #3 entrys in direct line. Total of 3 intersections lost.

An unintentional roof fall occurred on the 3rd east panel back belt line at 57+40 entry #4. The roof fall measured 30' long x 20' wide x 6' to 8' high. The fall did not affect ventilation or passage of men.

An ignitioin occurred at 23+76, #2 entry, E-15. The ignition lasted approx 2 seconds and was self extinguished by the sprays from the miner. There was no damage to equipment and ther were no injuries.

At approximately 6 AM a belt conveyor caught fire due to contact with hot clinker. The belt fire lasted more than thirty minutes.

A roof fall was discovered in the No 5 intake entry of 1 left off of 1 north. The fall is located 1 break outby spad no. 1504 & measures approx. 19 ft wide, 22 ft long x 10-12 ft high and involved an intersection. There were no injuries or equipment involved. The fall did not affect the passage of men or ventilation.

An ignition occurred at 25+17, #1 Entry, E-15 Section. The ignition lasted approx 5 to 10 seconds and was extinguished with a wash down hose on the miner. There was no damage to equipment and there were no injuries.

A roof fall occured in the 3 east panel, crosscut #58, in the pillar line between #5 and #6 entry. The fall mesured approximately 35'x19'x5'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.

An ignition occurred at 25+17, #1 entry, E-15 section. The ignition lasted approximately 10 to 15 seconds and was extinguished with a fire hose and rock dust. There was no damage to equipment and there were no injuries.

A roof fall was found by examiner doing pre-shift on 6/10/06. Fall was located at 80 cross cut between 5 and 6 entry of 5r MS. Fall was approx 20'X20'X10' high. Area had been mined Oct. of 2004 and was supported with 8'full resin bolts. No injury resulted but an intake stopping was covered up. No plans to clean up fall, props have been set and ventilation reestablished.

A roof fall 300' long 19' wide 8' thick occurred 600' outby the section. The fall occurred in the #2 return entry of #5 belt 60' outby spad 56074. 5' torque tension roof bolt supports was in use. No injuries occurred as a result of the fall, passage of men was not affected, ventilation was affected as 4 stoppings were damaged. Fall was due to laminated coal/sandstone streaks.

An unintentional roof fall occurred in the 7I tail-gate @ 38 X-cut. The fall measured approx 8'high X 12' long X 15 1/5' wide. No equipment damage or injury.

A roof fall occured in the 3 east panel, crosscut #58 in the intersection of #9 entry. The fall measured approximately 19'x19'x5'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.

An ignition occurred at 25+05, #1 Entry, E-15 Section. the ignition lasted approx 5 seconds and was extinguished with a wash down hose on the miner. There was no damage to equipment and there were no injuries.

A roof fall was found in the #2 unit's left side return. the fall measured 12' wide x 50' long x 6' high. The fall is in an area that doesn't effect ventilation or the travelway. This roof fall will not be cleaned up.

A fall of ground occurred in the 4-5 450 heading where roof bolts were in use. This was above the anchorage point of the bolts. It is not know when this occurred, however, upon discovery it was immediately reported to MSHA and a 103k order was issued.

Roof fall was discovered on the tailgate side on N-panel Longwall set-up at approx. 11:53 pm between spads 14690 & 14691. After investigation it was determined that the roof fall was above the roof bolt anchorage zone. An MSHA field office supervisor was notified at 12:33 am. No personnel were affected, no equipment or ventilation controls were damaged.

A roof fall occurred in the 3 East Panel at cross-cut #54 in the intersection of #8 entry extending halfway through the pillarline between #9 entry and #10 entry measuring approximately 85ft. x 19ft. x 5ft. The area was supported with 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause.

A frictional methane ignition occurred in the No. 1 entry of the 2nd East section. The miner operator was trimming the bottom on the right side of the entry when he contacted the rock mine floor with the bits of the miner igniting methane gas.

The cage was on the way down & was approx 100' from the station when ee noticed smoke. Upon inspection we found the bottom coil of a dual coal arrangement which activates the service brake release mechanism smoking. The varnish was burnt & smoking on the coil, but the coil was still operational. Both coils were replaced. The hoist was released at 1:05 pm.

Employee was discarding scrap stone into a wastebin and pinched his finger between the steel tub and a piece of stone. He crushed his knuckle, had a compound fracture and severed the tendon in the little finger of his left hand. The little finger was amputated to the second joint.

A stone slab was placed in an upright position leaning on a John Deere skid steer (tip of fork attachment). When two workers were splitting the stone, it split and the stone fell over landing on injured's left leg.

Operator observed a large stone stuck in oversize discharge bars of the vibrating feed hopper. Operator shut down the hopper & climbed up on to the discharge of oversize stone plate. Operator tried to break free the large stone by prying on rock with pry bar, lost balance and fell on rock pile.

Saw doctor on 6/12/06 with pain to hip area. Bacterial infection set in at rt knee, rt flank & rt hip. On 6/23/06 realized he had scraped his rt knee on a concrete block, which was the source of the bacterial infection. Off work for approx. 3-4 weeks.

EE was washing down fly ash with 1 1/2 water hose Hot ashes blew back onto his left forearm, wrist and hands.

The miner operator was cutting number 2 heading into a worked out area. The scoop operator was pushing material toward a newly created opening. The cutter boom of the miner contacted the bucket of the scoop producing a jolting action resulting in the scoop operator's injury.

After tramming the miner to a crosscut, the three man crew were placing the miner cable along the rib out of the roadway. As the mini-trac was pulling on the belt, he stepped on the belt, lost his balance, struck his head on the ground.

Report of Injury not received until 6/28/06. Employee was pouring Drano in clogged sink in lunch room. There was about 3 gallons of standing water in sink. Waited 5-10 minutes & then used plunger to try and free clog. Liquid in sink splashed him in face & eyes. Flushed eyes for approx. 30 minutes. Medcor transported employee to hospital.

Employee states that while fire bossing on E-belt, he crossed under the belt to check the pressure pump. While doing so, he jammed his head into the four inch waterline when he raised up on the other side of the belt. Note: Employee did not start missing work as a result of this injury until 6-13-2006 and returned to work on 6/16/06.

EE was hitting a metal dolly wheel with a hammer and got a piece of metal in finger. Required surgery & 1 suture. He is working on restricted duty.

The i/e reported that he bent over @ the waist to pick up 1 end of 2 bars and felt a pull in his back as he was straightening up.

Doctors report indicates sprain to right knee, Medial Collateral Ligament. He initially refused treatment, and about 11:00 Am he went to his foremans home and requested treatment. He was immediately sent to Amargosa Clinic, Amargosa, NV

While cutting a haul truck frame with liquid oxygen/propane torch, EE was instructed to cut the piece 75% through and then wait for the foreman to secure the piece before the final cut. EE cut 100% through, at which point the 6000lb started to fall. EE was pinned less than 5 min. as foreman placed bottle jack, to extricate EE. As soon as EE free, immediately called ambulance.
