Accident type, without injuries

Pellet Plant man elevator was recently updated to the new code. We are having an occasional problem related to the upgrade to the electrical systems. Problem was fixed and the elevator has worked good since after outside contractor repaired the problem.

While installing roof bolts at the start of unsupported face friction from the roof bolter wrench and a steel roof mat ignited an accumulation of methane.

A fire was discovered in the pulverized fuel bin at ~8:30AM. An employee extinguished the fire with a fire extinguisher. The local fire department was called. There are no injuries as a result of this incident. MSHA was called at ~9:00AM. Inspector arrived on site and conducted an investigation.

During routine maintenance the bull wheel bearings showed signs that they needed replaced. Hoist was fully functional...passed all required inspections. Bull wheel bearings were ordered and replaced on 6/1/14. All production ceased to avoid a large number of employees from being underground until bearings were replaced due to unlikely but potential issue with bearings.

A fire lasting approximately 15 minutes occurred on #3 Brookeville Battery/Trolley, 20 Ton Locomotive. After investigation, it was concluded that the probable cause was a worn cable jacket on a battery lead made contact with a ground lead causing an arc. The fire was extinguished with the use of hand held fire extinguishers. No reportable injuries occurred due to the accident.

While tramming a D-10R dozer onto the Wash plant feed coal stockpile the employee attempted to travel past a drawn down hole to the coal feeder. The side of the hole failed and the dozer slid into the hole. The dozer rolled onto it's side at the bottom of the hole. The employee was not injured.

An unintentional roof fall occurred in the 10WLW travel road between xcuts 36 and 35 at footage marker 4975 in the #2 entry. The dimensions of the roof fall is approximately 50'L x 10'T x 18'W. The roof fall was cleaned up. Roof fall is referenced as #31A.

Two employees trapped in T-1 elevator. One employee was in elevator when another called for the elevator on another floor. After second employee entered the elevator the doors closed. The car went down 3-4 feet and stopped. Elevator repair service was contacted. MSHA notified. Employees were freed after approximately 58 minutes.

The #4 shaft hoist controller malfunctioned causing the hoist to trip out. The electrical department was notified of the problem when hoist failed to reset. The electrician's removed and replaced the hoist controller thus fixing the problem. The hoist became operable at 7:55 AM, was tested, and returned to duty at 8:10 AM.

The elevator at the #9 portal went out of service at 4:15 am. There was no one on the elevator, no one trapped. The incident was caused by a bad wire from the hen house to the pit. Schindler Elevator service was called and the elevator was back in service by mid morning.

An unplanned ignition occurred in the Koepe (Intake) Shaft. Two employee were welding on an I-Beam at the shaft collar when the ignition took place. No employees were injured as a result of the incident.

A roof fall occurred at the G-2 Panel (#4 Entry, #7 Crosscut, Belt Heading). The fall area was approximately 130' inby the feeder and measured 16.5' wide x 60' long x 12' high. The area was bolted with 6' SuperTwist bolts, 12' CC cable bolts, and wide bacon straps every other row. No people, equipment, or ventilation was affected by this fall.

Upon traveling with inspectors a fall was found in entry number 6 between xc101 and xc102 of the North Main headings. Approximate distance is 58 feet and height was approximately 3.5 feet. The fall was not above the anchorage point but bolts were sheared off. The fall was unintentional and there are no plans to clean.

A fall of roof occurred in the 7th right panel off the 6 Southwest Submain at crosscut #9, entry #7. The fall measured approximately 25' in length x 19' in width x an unknown thickness. The area was supported by 6' fully grouted resin bolts.

There appeared to be a software issue with the Profibus for the HC/PLC. The system was repowered/rebooted and was up and running by 10:45 pm.

Their was a unplanned roof fall on #3 unit in #2 entry in the 4th north panel at spad (5+25/2).

A rock burst occurred in the 37-232 stope which damaged a 14.5'x 13' area of the stope resulting in rock being dislodged past anchorage of bolts. This occurred with blasting at the end of shift. No miners were in the vicinity. The damage from this event was not known until 5/27/14 because personnel were prohibited from entering area until 5/27/14 to allow area to stabilize.

Oak Springs hoist was taken out of service due to a malfunctioning brake on the brakeman car. The brakeman car had a damaged wire underneath the car on the outby end that caused one of the brakes to malfunction.

Highwall mining in Pit #9 in hole 8 panel 2. The cutter head became lodged around 400 feet. Working with MSHA a recovery plan was accepted and the lodged cutter head was removed.

The mine lost power due to outages in the area of the mine. The Production Hoist is powered by the power feed. All employee evacuated via the Koepe with no injuries or other issues. Power was restored by 3:00 PM the same day.

A roof fall occurred in the #7 entry 1 block outby last open x-cut in an intersection. The fall was 18'x18'x9'high. No people or equipment was involved. All sides were cribbed and dangered off. The fall will not be cleaned up. We are now installing 6'/12' cable bolts in intersections until we reach x-cut #251.

Operator is submitting this 7000-1 to abate citation #9009049 issued on 6/12/2014 for failure to report. The roof fall occurred over a pressured MRS in the #5 entry, directly inby spad 2071 on the MMU 004-0 that resulted in the piece of equipment being unable to tram itself free without assistance. Do not believe the roof fall meets the requirements as a reportable roof fall.

During routine inspection it was found that the slope car brake was not working properly. After further investigation it was found that a ground wire was broken. The slope car was removed from service and the ground wire was repaired.

A unplanned roof fall was discovered in 2B panel (11+40) XC#16 Entry#6. The fall was blocking the primary escapeway and had HV cable under it. Approx Length 18', Width 18', Height 6'.

There was a conveyor belt that caught fire and went to the insulation on the roof of the building. The fire flashed from one end of the building and put itself out. This fire lasted about 5-6 minutes. The fire dept. had been called but was not needed. Once the building was cleared of smoke employees went back to work and the building was back in operation.

At 9:00pm an ee was in Tilden Pellet Plant 2 elevator when it stopped functioning. The ee contacted a supervisor at 9:10 and advised of status. Electricians called to troubleshoot were unable to resolved the issue. Elevator repair service contacted. MSHA was notified of entrapment. Employee was able to clear the elevator at 10:50pm and then MSHA was updated.

5:50 pm security Officer noticed black smoke coming down valley, went to check it out. Found 992G Loader on fire. 6:06 pm called person from Raven Crest. 6:08 pm called 911 at time of arrival on scene fire was coming out around both back tires and up over the top of them. Didn't see any one around, Security Officer went back and waited at post 4 on fire trucks.

At 2:28am, power went out. Upon investigating the surface transformer they noticed the center phase had blown. Realizing power was not going to be restored, MSHA was called, and an evacuation was initiated. Further diagnostics determined it was an internal fault on the transformer that had been in service for 34 days. Transformer was removed from service and spare was installed.

8:15am the #3 Hoist went down.RexRoth card in the West Manifold Cabinet that regulates the pressures for the west calipers went bad.DHS was not able to see the pressures on the west brakes and would not reset.MSHA called at 8:30a. Mine Evac called at 9a. The card was changed and the pressures reset. Completed 2 test trips with no issues and then sent everyone back into the mine.

The #1 hoist drum bearing housing slid to the south preventing the mine from operating the hoist. The mine still maintained two means of egress with the #2 and #3 hoists.

Operator is submitting this 7000-1 to abate citation #9009048 issued on 6/12/2014 for failure to report. The roof fall occurred from a completed left-hand lift in the #8 entry, directly inby spad 2101on the MMU 004 causing a piece of rock to block the route of the MRS. Do not believe the roof fall referenced in this citation meets the requirements as a reportable roof fall.

An assistant mine foreman found a fall in the return at E-mains station spad 2333. The fall was at an intersection measuring 18' x 55'. The height of the fall was 9'. The area was dangered off and supported.

An unplanned roof fall occurred in the 4th Panel of The North west Mains at spad 1350--The fall was 16' X 18' --4 1/2 ' Thick. The has been dangered off at all approaches.

At approximately 9:10pm the feeder helper ee was traveling in the passenger elevator at the cement silos. The elevator car stopped and the employee was inside the car for more than 30 minutes without the elevator working.

An unintentional roof fall has occurred 7 breaks inby surface at spad 3939. The roof fall was approximately 100ft x 18ft x 5ft. There were no injuries reported and no equipment involved.

This mine has experienced an unplanned roof fall in the Main North Return. Location was at cross cut #-7 in the #2 Entry. (202+70) The fall was approx. 20ft long X 20ft wide and approx 15ft high. there were no injuries, witnesses or disruption to ventilation.

An inundation of water occurred on the No. 1 unit in the No. 2 entry at x-cut left of MMU-005-0 section at spad No. 11 + 09. The working section is presently under a 103-K order issued by MSHA on 5/14/14 at 0036 hour.

Brake fault on right brake #3 hoist would not release(brakes were set). Evacuation called at 11pm. Msha called at 11pm. 18 Hourly, 2 salary evacuated from #1 Hoist. Electrical supervisor reset hoist. Two complete test trips were completed. No problems found after reset and test trips. 12:25pm hoist was fully operational, evacuation called off and third shift crews went UG.

At approximately 1:45 pm on Tuesday, May 13, 2014 a brush fire was reported at the Lompoc Mine. The brush fire occurred in a vegetated part of the property, but not in an active working area of the mine. The origin of the fire is still under investigation. The facility was evacuated safely and no persons were injured and no structures damaged as a result of the fire.

At 11:50 p.m. the roof fell in the #4 room between spad #'s 2+60 and 3+10. the fall was approximately 18 feet wide 35 feet long and 6 feet high. The area was cribbed off and the area dangered off. The fall did not impede ventilation.

The mine hoist failed to operate due to blown fuses. Called MSHA hot line at 10:52 PM. The fuses were replaced and the hoist ran through all test and put back in service at 12:00 AM.

An unplanned roof fall occurred #5 Belt one break inby spad #414. The fall is appx 20' X 20' above anchorage. The fall was dangered off at all approaches. Six foot fully grouted bolts used. Notified the agency in the required 15MIN , Local agency also notified.

A roof fall occurred in the 1st main west at # 5 neutral entry at spad # 55+89. The fall measured 55'x19'x10' and was caused by a hidden slip in the mine roof. The area was supported by 5'fully grouted bolts and 10' cable bolts in the intersection. The area was mined in March of 2014. There were no injuries, no equipment involved and ventilation was not affected.

Number 9 elevator went out of service at 1:40pm. No one was on the elevator, no one Trapped. Elevator was up and running at 3:00pm. They believe it to be caused by a switch Leveling Switch.

Small brush fire was observed. At the time it was observed and called in, it was unknown if the fire was on mine property. Within a few hours it was determined the fire was not on mine property. Cause is still unknown. Was put out by approximately 4:10pm on 5/9/14.

An unplanned roof fall occurred in the 1L mining section at 42 wall. The fall measured approx. 45ft long by 16ft high by 16ft wide. The fall occurred in the belt line, #1 Entry. There were no injuries or equipment damaged as a result of this event.

At approximately 2:08pm the kiln operator received an alarm that the East and West explosion vents had opened on the #2 kiln coal mill system. After inerting the system and required safety checks were completed an internal inspection was conducted. Some accumulations of coal was found around the inside of the explosion vent framework.

A fall occurred on the #8 Main Belt, #13 Crosscut. The fall area was bolted with 8' install two and 12' CC cable bolts with straps on an alternating pattern. No people, equipment, or ventilation were affected.

While mining with Cat HW300 highwall miner we started having communication problems with the cutter head. While extracting the cutter head it became lodged in the hole. A recovery plan has been accepted to retrieve the miner.

At 19D 1A-1 4 block charging area two employees went to put a 650 scoop battery on charge. Mistakenly hooked up a 636 ""Y"" jumper to the 650 batteries, shorting the batteries. Sparking and a small flame occurred for less than 10 seconds until extinguished with a handheld fire extinguisher. Process continued a dozen times until the battery leads were cut at approximately 10:55 am.

A roof fall above the anchorage was discovered on the 8 East longwall face near the #1 Shield Location. The fall measurements were 20' long X 16' wide X 12' high. Travel was affected through the area.

Elevator at McMahon portal when the elevator landed at the top the doors would not open. Starting at 12:50 am and were opened at 1:40am. A door fault was found and repaired.

An unintentional Roof Fall has occurred on the #1 Section #2 Face spad 10358. The fall was approximately 30ft x 18ft x 24"" to 28"". There were no injuries reported and no equipment involved.

A fall of roof occurred in the 2nd Right panel off the 3rd Southeast Submain at crosscut # 28, # 3 entry. The fall measured approximately 18 feet wide x 18 feet long x 6 foot thick. The area was supported with 5 foot fully grouted roof bolts.

One miner was entrapped on the Sugar Run elevator, one quarter way up from the bottom, from 12:35 pm to 2:19 pm. A door switch had an electrical fault causing the brakes to lock because the electrical circuits showed an open door. The switch was replaced. Schindler elevator services was contacted immediately.

An unplanned roof fall was discovered by the weekly examiner. This occurred in the 10th right entry 1 between x/c's 7 & 8. The roof strata was comprised of silt stone, carbonous material and unsolidified mud. Fall was below the anchor zone of the roof bolts and material fell from around the bolts. This was the secondary escapeway for inby sections.

The rolls where adjusted to far out and the chute had become plugged. There was no external or internal damage. To correct the problem the rolls where adjusted and the chute will be monitored to insure the chute does not plug after the rolls are adjusted.

An unintentional Roof Fall has occurred on the #3 Section 2nd panel off 4 left spad 10180. The fall was approximately 140ft x 20ft x 8ft. There were no injuries reported and no equipment involved.

A fire was noticed burning in a wooded area that had been reclaimed. The local fire department and forestry service responded to the fire. The fire was contained by 1845 HRS that evening, but there was still many areas smoldering inside the fire break. The fire was monitored over the weekend and was completely extinguished on May 5th. The fire burned approximately 5 acres.

A breaker tripped on the Dynapar braking system for the Galena shaft hoist. Electricians were called in to determine the reason that the breaker tripped. It was determined that the control card for the system needed to be replaced. The repairs were made and the hoist resumed operation at 6:20 P.M.

Roof fall was discovered by the weekly examiner in the 6th NW Submain XC# 117, Spad 84+10, Entry # 2. Approx Length 40', Width 50', height 4'.

Caught in, under or between a moving and a stationary object

The victim and another miner were tramming the roof bolter outby down the No.3 entry of tailgate No.1 when the accident occurred. The victim was injured when he was struck by the roof bolter as he placed himself between the coal rib and the roof bolter.

Fall from machine

During maintenance, a miner was struck by the boom of an excavator that was being used to position crusher parts; causing him to lose balance and fall approximately 11 feet. The fall resulted in several broken bones. MSHA arrived on site the afternoon of the accident and started an investigation. *Employee died on 5/26/2015.

Struck by falling object

On 5/12/14 an event occurred at this mine which resulted in the entrapment of two (2) miners. Upon subsequent recovery it was determined that both miners had perished. The injured were fatally struck by coal/rock. An investigation is on-going. The causes are unknown.

The victim was unloading a rail car using a transloading conveyor and was entangled in the feed end of the conveyor.

No eye witness to accident. ATV was traveling up a + or - 15% slope and lost traction. The road has some uneven spots that might of contributed to the loss of traction. The ATV appears to not have been in All Wheel Drive which could have contributed to the loss of traction.

Using a spud bar to clean out a chute on the S2, reaching in with the bar through an access panel. Bar slipped and right hand was caught between bar and the edge of the access panel. A severe avulsion of the fifth finger from the second knuckle and a small abrasion to the fourth finger on the right hand.

Employee was working with another mechanic and was lining up a safety pin for the tailgate of a haul truck and his index finger was pinched between the end of the pin and the under side of the haul truck bed crushing the tip of the finger.

Employee was watching the V-Belts in operation on a motor that operates the shaker for the Main West bin. After de-energizing the power he felt the V-Belts to determine their tension. Immediately after making contact with the V-Belt the shaft rotated and his finger was caught between the V-Belt and the pulley. The shaft then immediately returned releasing his finger.

Employee was adjusting the air flow on the green float bank B-Cell when the middle finger on his right hand got caught between a moving paddle wheel and the guide frame for the tee handle used to adjust the air.

The employee was cleaning the rotary vane valve unit on a bag house discharge. The employee placed his hand into the vane valve housing, at which time the unit activated, rotated, and caught the end of a finger on his left hand, causing a crushing avulsion.

3 employees were assembling conveyor sections to extend the field run. One section was not lined up properly. An employee attempted to pull it into alignment by hand & felt pain in his abdomen. He didn't immediately seek treatment. He later saw a doctor & was diagnosed with a hernia & will be scheduling surgery. He reported the injury on 5/23/14.

employee walked behind 1-80A exhaust port, got curious about air coming out of exhaust port and stuck left hand in port far enough to contact powered fan wheel. employee suffered amputation of middle finger above 2nd knuckle, shattered bones in 1st finger requiring finger to be fused at a later date and lacerations on 3rd and 4th fingers.

REPORTABLE ONLY - Employee had sit down in shuttle car and had fingers in the grating (used for a guard between operators compartment and the boom of the car) adjusting seat when he accidentally lowered the boom of the car mashing the end of his finger between a rock hanging on the boom and the grating causing a crush injury with partial amputation of right index finger.

Employee was pushing a roll into the roller mill with port-a-powers. As employee released a port-a-power to insert a spacer the roll came back pinching three fingers on his right hand causing lacerations and fracture.

Employee was assisting to push a roll of belt off of the winder in the 1A storage unit when he felt pain in his left knee. He was later diagnosed with a torn meniscus that required surgery to repair on 7-24-14.

As he was dismounting the machine he felt a pop in his right knee when he placed his right foot on the ground. The employee worked regular duty until June 26, 2014 when an MRI indicated a fractured bone at the end of the femur lopes on the upper part of the knee.

EE turned the small coal sieve to retrieve any magnetite from the water where coal had been washed. This sieve rests on a pivot stand and is turned weekly. Employee was checking an air operated gate on top of storage silo 2 gate 7. Employee drained moisture bowl and noticed a smell of ammonia. Employee was exposed to ammonia that got into the compressed air lines from the ammonia injection system which had been started earlier in the day. The ammonia injection system was turned off at the time of the exposure.

Employee was changing speed reducer on a shuttle car. The speed reducer slid off the forklift and fell and hit his left middle finger - cutting it. He received 8 stitches.

Employee was working on a D10 Dozer he had a 6 foot ladder leaned against the push arm. He started to climb the ladder and was about five feet off the ground he said the ladder tipped over with him. He fell hitting the steps and the push arm and falling to the ground. He said he was all right. later it was found that he had broken ribs punctured lung and punctured spleen.

Employee stated that while roof bolting on the left side of the Sandvik continuous miner on the 9 Right section the roof bolt that he was inserting into the hole started to bend. Became reportable on June 12, 2014 due to fracture. *EE began losing time on 6/30/14 due to surgery.

A loader operator twisted his left ankle when dismounting the left rear steps of a Cat 988H wheel loader. There was loose rocks near the bottom area of the ladder that he potentially stepped on. The loader operator had gone to the cab for his lunch box for an early lunch break.

The employee did not realize it had happened until 20 minutes latter when he was washing up at the end of his shift. This caused a small chemical burn on his L forearm and R elbow.

The cause of the incident was poor hand placement.

Employee was attempting to fuel haul truck, had pulled on the fast fuel hose trying to make it lock when it jerked her towards the reel, pulling her shoulder area. She reported her shoulder hurt 2 hours later, was taken to the ER, diagnosed with cervical strain, and taken off work. Currently ONGOING.

He is a trained supervisor with 14 years experience in this role. He was operating a trolley/battery jeep which he is trained to do and has over 30 years experience in jeep operation. He was wearing all personal protective equipment. His trolley pole came off the wire. As he attempted to put the pole back on the wire, his left wrist contacted the energize wire.

No issues with training, equipment malfunctions or mining methods.

He stated that it came out of gear & truck started to gain speed. 2 employees in 2006 Cat 735 seat belts fastened. Trainer was training employee on driving haul truck. The trainer w trainee driving 3rd load. Going down a steep road in 4th gear, truck jumped to 7th gear & speed increased w brakes on. Going too fast on curve, flipped onto its side. Both Employees have minor injuries. Investigating root cause of jumping gear.

During maintenance, parts on the conveyor were replaced. The belt was started up at low idle for tension and tracking, with tail pulley unguarded. A labors arm was entangled in the smooth (none fluted) tail pulley, causing cuts, nerve damage and broken bone (s) injury. The injured labor was taken to local hospital where he was treated before being medevac'd to Seattle.

Employee was working in water and grout from the drill hole. Employee sustained 3rd degree burns and remains off work for an uncertain period of time.

Employee stated that he was working along the 3-B belt when a piece of rock fell from the top and struck him on the back of the neck. The rock was approximately 22"" wide and 0"" to 1"" thick. This resulted in an abrasion and one lost work day. This incident became reportable on 5/22/14.

A contract employee was standing in the intersection of # 1 entry hanging tie wire to hang the continuous miner cable. The continuous miner was parked between #2 and #1 entry. The continuous miner operator started tramming the miner to #00 entry. When the continuous miner entered the intersection, the contract employee was struck by the head of the continuous miner in the leg.

The employee in question was working to weld a plate onto an overhead location. The employee had pre-positioned the plate against a ladder that was beside the ladder he was to ascend, the employee then reached down to grab the plate, and the plate began to slide, and was unable to catch the plate to stabilize it, causing the plate to strike the employee's left foot.

When he was getting back on the dozer, his foot was on the step on the push beam and his hand was on the hand hold. While changing batteries in Fairchild Scoop, lifted battery lids and latched doors, door got jarred causing lid latch to slip and fall on right hand resulting in a fracture.

Employee started up ramp with loaded haul truck when the truck shifted from 1 to 2 gear, 3-4 times. Employee didn't seek medical attention at the time of the incident but did ask to see a doctor on 6/09/14. The hard shifting resulted in the employee suffering from cervical strain/spasm. He was issued restrictions on 6/10/14 from a follow up visit, making this a recordable injury

Employee went to the top floor of Dry Plant L to check the bin levels and tripped when entering the doorway. He lacerated the left thumb on the rotex during his fall. The rotex machine was inside the doorway.

3 employees were changing 2 worn Jaw Dies. After successfully removing both and replacing one, hooked up second jaw die to lift in place when chain hook slipped out of lifting eye and jaw die overbalanced and fell on one miner's leg. The 2 supervisors quickly used crow bar to free miner. The miner was immediately taken to the ER and it was discovered his knee and leg was broken.

Employee stated that he was putting a 10 inch pipe clamp on the return line to the heavy media cyclone sump. As he was installing the clamp, he was reaching overhead and in an awkward position when he felt pain go down his right leg. Went to Doctor to get checked. RTW with some restrictions.

Employee was pulling down on the elevator door to close it and it came down easier than he expected, pinching his left hand between the two door sections.

Laying rail in 23D, moving the rail with a slate bar 18"" butting it up to the installed rail to install a fishplate and felt a pain in the lower stomach/groin area. Diagnosed with hernia on 6/2/14. No issue with mining machines, methods, PPE or compliance.

Employee was driving Haul Truck and felt a pop in his right shoulder.

Employee was putting on rubber and metal plates on wash plant while standing on the main feeder belt and eco block. Foot slipped off the main frame and eco block and employee fell backward onto the Grizzly motor frame. Employee did not go to Dr/Hosp until 5/31/14 with soreness. No broken bones, just bruised

Employee was replacing an air pump in operators seat. He had the seat blocked up and the block slipped crushing his right arm.

Employee was flipping a stone on roller table to push thru to chopper. When employee flipped stone he hit his right pinkie finger between stone and roller. Broke tip of pinkie and took chunk of pinkie out from under the finger nail.

Employee injured his right thumb while making an adjustment to the forks on an off-road forklift in preparation for a lift. The fork came off of the support saddle and swiveled pinching the employees hand between the base of the fork and bottom part of the carriage.

The Ground Man attempted to dislodge a rock from tail pulley and was caught by the conveyor belt. The Ground Man was pulled into the tail pulley by his left arm, resulting in a broken left arm.

Employee sustained fractured rib when come-along hooked to air compressor shifted making contact against his left rib cage.

Emergency Services Officer was conducting his pre-shift vehicle inspection. While walking near the squad he tripped over a curb. He extended both arms hands out to break the fall. After landing he felt a pain in his left shoulder and left thigh.

Employee stumbled over a floor drain and hit his left shoulder on the wall. It was later determined that he would need surgery for a rotator cuff repair. Note: surgery date of 8/1/14.

Employee was dragging de-energized high voltage cable to the 5b belt head kva. As he was walking with the cable over his shoulder, he slipped and tripped causing him to fall and fracture his right lower leg.

EE was unloading a practice mannequin when he felt his leg buckle under the weight of the mannequin (approx. 150 lbs.). EE stated he heard his leg pop. EE was checked by first responders and taken to the ER. EE was diagnosed with a fractured Tibia and taken off work and referred to an Orthopedic Doctor.

Employee stated he was walking the belt entry back to section from locking out the belt head when a piece of draw rock fell from the roof and struck him on the neck and back resulting in a strain of the neck and back. This incident became reportable as lost time injury on 5/20/14 due the employee missing 1 day as a result of the injury.

The employee had entered the spare parts trailer and was performing clean-up. When he exited the trailer he was carrying an empty cardboard box. He walked down two steps, lost his footing and fell. He placed his right arm out while falling resulting in a right elbow dislocation.

As the bucket approached its intended position, IP placed his hands on the gate handle with the intent to pull down handle to dispense cement. The IP had started to pull down on the handle when his hands slipped off the handle. The handle sprung back with force striking the IP in the lower jaw and chin.

An employee lifted a chock block to place it back on a fuel truck. This action caused him pain in his right arm. No medical treatment other than ibuprofen was given. The employee stated that the pain never went away and it was looked at by a doctor on 8/5/14. At this time the employee was given work restrictions and surgery was scheduled. Surgery was done on 8/13/14.

At approximately 7:20 a.m. on Tuesday, May 13, 2014, an electrician was in the process of pulling wire through a 3/4"" electrical conduit from a junction box to the belt scales. As he pulled, his left foot slipped and he started to fall but was able to catch himself and regain his footing, but he twisted as he did, and he felt a pop in his back.

After using the restroom, the operator noticed one of his chock blocks was falling out of the holder. He readjusted the block into the holder and felt some pain in his right collar bone and neck area. Reported to supervisor. Seen at doctor on 05/15. Confirmed work related on 05/28/14. Diagnosis was right shoulder strain.

2 miners were installing a loader bucket liner. During the process while tack welding a section, the bottom of the jack slipped out of place striking the ee in the ribs. All ee involved were wearing the required PPE. The ee were truing to take extra precaution by eliminating any slippage of jack near the top of the bucket by welding on a strip of steal for the jack to brace against

Fall from machine

Operator exited the cab of the crane and walked across the deck toward the ladder to climb down. Just as he approached the ladder, he slipped and fell to the ground landing on his feet and then buttocks. Employee was taken to hospital with complaints of pain in his right ankle/foot. Xrays revealed a fracture a small heel bone that required a cast to heal.

Officer was exiting the porta-pottie, behind the Main Gate Entrance at Barrick Goldstrike. She tripped, lost her balance and fell to the ground. Officer landed on both knees and hands, jamming her right arm. She finished her shift and once in town, taken to hospital for further exam. She fractured her right elbow.

Employee stated that he was making repairs to a diesel mantrip. After lifting the rear end into place he felt a burning sensation in his abdomen and groin. Medical received on 6/19/14 diagnosed the employee with a hernia.

Employee stated that he dismounted the RBM to man the drill station and as he stepped forward he tripped on a rock. As he fell, he extended his left arm and his hand struck the drill pot. Medical received 05/09/2014 states a fracture to the base of the fifth finger.

2 employees in 2006 Cat 735 seat belts fastened. Trainer was training employee on driving haul truck. The trainer w trainee driving 3rd load. Going down a steep road in 4th gear, truck jumped to 7th gear & speed increased w brakes on. Going too fast on curve, flipped onto its side. Both Employees have minor injuries. Investigating root cause of jumping gear.

Employee was throwing the miner cable over on #3 unit in #6 right when he felt a sharp pain between his shoulder blades. He continued to work till he was put off work for therapy on 5-19-14.

The doctor prescribed OTC pain medication along with icing injured shoulder.

Employee was drilling an entry round to a stope from inside the #4 Raise-South Side. While drilling, a 3"" x 12"" x 5' piece of lagen fell from a higher point of the raise and struck him on top of the left foot. The result of the impact was two broken bones on the metatarsal part of the left foot.

Cleaning inside of 8 washer liquor tank on the 4th deck with a water hose. Caustic mud splashed back on EE while cleaning the tank (all appropriate PPE was employed). EE began cleaning hard hat and face shield while still donning hard hat and shield when he removed goggles to clean face, at this time mud fell into eye.

Miner was stepping down off of Kubota 5 in the 1908 stope access. Miner dismounted in the front facing position, left leg first using 3 points of contact. After reaching the ground with left leg, miner fell to the side losing balance, feeling immediate pain and heard ""popping"" sound.

On May 21, 2014 at approximately 6:30PM, employee was pulling a swing block out of the conex building just East of the bucket wheel. He reached in and pulled it out with his left arm. EE hopped out of the loader and fell on the ground hitting his elbow on a piece of stone. He went to the emergency room - had x-rays taken which showed a previous bone spur had broken off. He went to the emergency room 5 days after on 5/21/14. Did not leave work on the day of the injury.

An employee was removing Bucket Bits from a loader, as he was removing the nut from the plow bolt the bolt pushed out of its recess causing the bolt to spin. The employee put his hand on the head of the plow bolt and tried to remove the nut, at that time the bolt spun again causing the sharp edge of the bolt head to cut through his leather glove lacerating his finger.

EE stated that the 1 North section belt head had went off on gob and he stepped up on the side of the flow-thru to see inside of the rock box. As he was stepping down he grabbed a cinder block that was sitting on the flow-thru that gave way and caused him to loose his balance and fall against the mine floor and onto his lower R leg. Diagnosed w/fractured leg, on restricted duty.

The employee was changing oil while standing on a catwalk approximately 10FT above ground, employee turned and foot slipped causing the employee to fall on the catwalk. The employee struck against the electric motor causing a strain to middle back and rib cage. The employee is seeking medical attention.

Struck against stationary object

Employee was carrying water hose, while crossing a roof bolter cable it took up slack causing the cable to whip, catching the EE causing him to trip and roll his ankle. Injury was not determined to be recordable until the employee returned to his regular scheduled duty, and was put on restricted duty because of his injury.

Inhalation of radiations, caustics, toxic and noxious substances

Refiners were cleaning the mercury scrubber after it had been flooded. Two refiners opened the access door on the side of the scrubber to clean/remove residual water. Upon opening access plate, both refiners were exposed to SO2. After the initial exposure, one of the refiners continued to feel ill while on his days off and went to the hospital to be examined.

Employee was running the miner making the initial cut in #2 to #1 x-cut. A small rock (less than 10 pounds) struck his hat then slid down his face cutting his cheek and lips on the left side of his face. The rock was very sharp. The area was fully roof bolted.

Drill helper was hand threading drill rod onto drill string. Driller moved rotation unit down before drill rod was threaded and helper was clear. Rotation unit caught drill rod forcing it down pinching drill helpers foot between drill rod and drill deck.

Moving miner cable out of the roadway tripped over the miner cable and jammed left little finger. (EE worked from 13th to the 16th before going to the Dr. to have his finger checked. He went on lite days on Monday the 19th and return to full duty on the 27th)

EE was shoveling material and felt a ""pop"" in his back, he continued to shovel until his back pain became intolerable. EE was seen by a physician and diagnosed with a lower back strain.EE has been placed on restricted duty and prescribed medications to treat symptoms. Follow-up Scheduled for 5/16/2014.

Employee stated he jumped out of the service truck he was operating and stepped on a rock with his left foot. He felt a pop and fell to the ground. He was later diagnosed with a torn ACL and underwent surgery on 05/30/2014.

Employee was walking on the motor side of a belt drive and stepped on a rock and twisted his right knee. Employee finished his shift.

Employee alleges that when he raised the face shield on his hard hat, dust got behind his face shield and safety glasses and got into his left eye.

EE was assisting with replacing hoist motor on 24 shovel. As EE was walking to get a wrench he stepped on a rock twisting his left ankle. EE was taken to the clinic to be checked he was placed on restrictions and release. EE went on 05.13.2014 for follow up with doc and given a full release back to work.

Employee stated that while assisting co-workers to belt up the section, he lifted a piece of belt structure straining his left shoulder. He continued to work his regular job until he had surgery to repair a torn rotator cuff on 12/2/2014.
