A push button control relay to the hoist was showing a fault. The mine was evacuated until repairs were made. The hoist was restored to service at 7:25 a.m. The hoist was tested and miners returned underground at 7:40 a.m.

Roof fall was discovered by examiner at approx 2:15. He then contacted the RP who traveled to the location 1st NW Mains between XC#126-127,spad #104+37 to 105+27. Once the RP arrived investigated the fall, collected the information needed for the questions ask per MSHA. He contacted outside to make the call at 2:40.Approx Length 35',Width 19',Height unknown.
Employee spotted smoke coming through the woods. Employee followed the smoke until he came up on the fire near the Mt Vernon Cemetery off of CR 216. Fire Department was called to the scene to put the fire out. The fire is believed to be caused by lightning strike yesterday morning. The Texas Forestry Service was called in to trench around the fire.
There was a slough that occurred in Pit#24 about 250 feet east from the active pit. A Komatsu 490 track hoe was sloping the area when the slough occurred and partially buried the track hoe. Under the direction of MSHA a plan was implemented and the track hoe was removed with little exposure to any miner.
The graveyard supervisor was making his rounds checking on the stopes when he found a portion of the back had fallen out in 6250-15 West stope. No one was working in the stope at the time of the failure.
Main hoist brakes would not release. Hoist went down at 8:21 am. Mechanic and electrician informed management that hoist would not be repaired in 30 minutes. Call was made to MSHA hotline at 8:42 am. All electrical and PLC functions were normal. Cleaned brake shuttle spool and check valves. Replaced ""O"" ring seals in brake solenoid. Hoist returned to safe operation at 10:45am.
Elevator at #9 was on bottom and would not come up due to outer door lock failure. No one was on the elevator at the time of failure. Elevator was back in service at 10:45am
During a scheduled inspection, a kink in the slope hoist rope was discovered. The hoist was taken out of service and the rope was re-terminated. No injuries as a result of this event
Hot rock had fallen onto a stationary belt causing it to catch on fire
The #10 Contactor in the Hoist House (Surface) was faulty and needed to be replaced. This exceeded the time limit and the mine was evacuated due to only one escapeway available. All miners evacuated with no injuries or other issues. The Hoist was returned to service around 2:00 PM
Damage to the #1 hoist drum bearing housing prevented the mine from operating the hoist. The mine still maintained two means of egress with the #2 and #3 hoists.
The Enlow Hoist was taken out of service because the scatterhorn was not functioning. There was no damage done to the hoist. The hoist was still functioning properly.
A roof fall was discovered in the 6-North Right return at Spad # 7536. no persons injured, ventilation not disrupted, walkway is obstructed. Roof will be supported a walkway reestablished.
West loading pocket of the production hoist broke free at the flange connecting it to the butterfly gate. The pocket slid down damaging a cat walk and one of the nearby button timbers. The cat walk has been removed and the button timber has been cleared from the hoist way.
There was a unplanned roof fall on 1-54 neutral at cross cut 37 & 38.
A roof fall occurred in the NW-B section (MMU 001-0) in #1 to #0 crosscut between survey station 7219 and survey station 7185. The fall was approximately 50 feet long x 20 feet wide x 8-10 feet high.
Found roof fall in the Joe Day Main #6 entry at 1:06 PM. The roof fall was approximately 20'w x 20' long x 10' high. All approaches have been dangered off and additional roof support has been placed in front of the fall.
Equipment started on fire. Cause is undetermined. Engine compartment of equipment was severely damaged. No injuries occurred.
Had an unintentional roof all in the #3 Heading between breaks # 13 to 14 on the 4th Left panel off of Pratt Fork Submains.
Slack Rope Cable Fault-Too Low. No one trapped at time of incident. Elevator was up and running at 7:20 am.
The Dry Ridge Elevator was not operational for an extended period of time. There was a fault on the car govenor slack rope. No one was injured as a result of this event.
The Crabapple slope hoist was not operational for an extended period of time. This event was scheduled to replace the existing slope cable with a new slope cable. This slope hoist is not used as any type of escape way.
Contract coal hauling tandem axle truck left scale house road loaded and after entering haul road to his right continued to veer right driving into drainage ditch and turning truck on right side.
a roof fall was found in the #7 entry of No. 1 Southwest Mains 1 break outby spad #607. This is not a regularly traveled area. The fall was approximately 20' long, 20' wide and 5' thick. The area was timbered off.
Hoist car was down due to mechanical problems.
A blaster for sub-contractor, Austin Powder, left his truck in reverse gear and let truck roll off 14-16'high embankment. No injuries, removed truck and drove out of mine site, attached to wrecker, and went to Austin Powder shop.
Slope hoist was down due to mechanical problems.
Rail mounted trash car caught fire at 123 block on the Main North haulage. Communications were disrupted until 10:50 a.m. Trash car was covered with a fire retardant cover. Fire was extinguished by fire extinguishers, rock dust and water. Ignition source unknown.
An Unplanned roof fall has occurred in the #2 entry/Spad #140. The fall was approx. 20' X 20' X 6' above anchorage. The fall was dangered off at all approaches. Local agency notified also.
Rock fall in #1 entry approx. 20' inby spad 156 through intersection to 10' inby the inby block.
The D.C. circuit breaker to the service hoist drive motor knocked and would not reset. The hoist was examined and the service hoist was put back in service at 8:30am on 04/15/2014.
While doing a weekly examine a rock fall was found at Shell Branch Bleeder in the #2 entry at brk. 25-26. The size of the fall was approximately 4 to 5' thick 20' wide and 40' long. The ventilation has not been affected, cribs are being installed in the affected area.
There was a loose wire in the Production Hoist controls and not all lights were available to run hoist. All employees evacuated with no injuries or other issues.
A fall of the roof above the anchorage was discovered in the crosscut between the Main East Track and the old Empty Track at 10-30 crosscut carrying approximately 5 feet into the Main East track entry. The fall dimensions are 15 feet wide by 50 feet long by 13 feet high. Travel was affected in the Main Track Entry.
An unintentional roof fall occurred in the 17H section at 1.5xcut between #1 and #2 entries. The roof fall was approx 60' long, 12' high, and 16' wide. There was no equipment damaged or injury as a result of this incident. This fall will not be cleaned up.
A Security Officer noticed smoke coming from an unknown origin. Upon further investigation the officer found an active brush fire. The fire was extinguished by the fire department. No equipment was running as this was a Sunday.
A roof fall was discovered during the on-shift examination by the section foreman . The fall was above anchorage and did not impeded ventilation. The location of the fall was at survey spad number 660 in number 4 entry.
East Finley elevator broke down to the brake pick needing adjusted. No one was abroad the elevator at the time. The elevator was at the bottom when the incident occurred.

A fire was noticed burning in a roll off trash container near the remote shop. The fire burned for approximately 35 minutes before the local fire department was able to extinguish it. The trash container received some minor damage due to the fire.
Two employees stepped in elevator. The doors on the elevator closed but didn't operate. Another employee was alerted of situation. Arrangement made to contact repair service. Employees able to clear the elevator about 6:10 , after being stuck for approx. 40 minutes. MSHA was notified of the entrapment about 5:50 then updated at 6:15.

While building a brattice line, a small roof fall was discovered that had occurred some time in the past. I was in an area that was mined more than 20 years ago. It was not in a regularly traveled area. The fall was approximately 50' long, 20' wide and 3.5' thick. Located in #2 heading in #1 Mains i break inby spad #337.
EE was going up the elevator and it stopped in-between floors. The up normal and bottom final contacts were found to be dirty causing the elevator to stop.
EE was mucking out of 2 tunnel in the b/t of U62 in F intersection. Large slab of roof rock lying on top of the muck pile. As EE was mucking he noticed the slab sliding down the pile so he proceeded to back out of the entry. At that point the EE noticed the roof started falling past the last line of bolts and continuing into the bolted ground so he took cover in the loader.
An unintentional roof fall occurred at 25 cx #3 entry, spad #58 on the Cedar Grove 3 South section. The fall was 12 ft. long, 19 ft. wide and 6 to 8 ft. high. A plan was submitted and approved to clean up the fall.
ROOF FALL - Occurred on the No. 5 section in the No. 1 face. The roof fell above the anchorage of one roof bolt of the last full row of roof supported. The fall was approx. 5 feet wide, 16 feet high and approx 18 to 20 feet in length.
A non injury roof fall was discovered at break #10 on the #6 belt in the #1 entry. It is app. 75ft long and 10ft thick and 19ft wide. The effected area is to be cribbed off or timbers. While doing a weekly exam.
An unplanned roof fall occurred in the Wyoming #2 mine, 1 brk. inby spad #870 in the #6 entry, that measured approximately 20' wide X 70' long at or above anchorage. No injuries were reported. The area was developed on 5/13/10.
A roof fall occurred in the intake entry at spad 279. Fall was approximately 20' by 50' by 6' high.
This report is being filed as part of a msha citation that is currently being contested. So I have to file this report so msha will not shut my coal mine down . There was an weather anonomaly that deposited over 5.5 inches of rain fall that compromised or return airway after we had withdrawn the miners 2.5 hours earlier and overwhelmed our dewatering pumps.
Hoist was taken out of service due to a kinked rope (4/7). It was fixed and tested (4/8). The car was pulled to the top and car pulled past top limits before hitting switch and final limit switch that failed to stop car. Car was pulled into the guide roller at the end of the track. *The brake coil was replaced on 4/5 while the hoist was under the supervision of Frontier Kemper.
A motor was pulling a lowboy into the mine. It came off track at 16 xcut on Cool Hill #5 and struck a overcast. The overcast partially collapsed due to wall of overcast being pushed out. The trolley wire developed a high resistance catching some wood on fire. The fire was put out and lasted 12 minutes.
While employee was performing the preshift of the mine, he came across a roof fall in number 2 entry at spad # 5011 aprox 3 x cuts outby. The section was in the process of moving back away from this area the day before.
Maintenance personnel discovered a compressed air valve turned off to the clutch pump. This valve was inadvertently turned off earlier in the evening while performing hoist maintenance. hoist was down from 12:15 am to 3:40 am.
The #1 elevator was removed from service at noon for scheduled maintenance to replace the car floor. A new floor was installed. The car was returned to service at 7:58am on 04/06/2014.
Traffic was entering 1E and exiting 1S. 1W was closed 1N was open. Highwall material sloughed onto canopy causing a rod attaching the canopy to fall blocking part of the portal making it unsafe. The mine was evacuated. Miners were instructed to enter 1N and exit 1S. 1E was closed. Mining resumed.
An unintentional roof fall was discovered 12:00pm, No Person or Equipment involved. In the #2 Entry-spad 267 on 1st North Mains in the Return. The fall was appx. 20' wide X 25'-30' long and 8' thick.
The service hoist tripped out while staged at the mine landing. Trouble shooting began and a bad magnetic switch was identified as the problem. The switch was replaced.
An unintentional roof fall has occurred on the Horse Creek #5 Belt and Track outby spad 4288. The roof fall was approximately 40ft x 30ft x 6 to 8ft. There was no injuries reported and no equipment involved.
The electrical switch that indicates the brake position was out of adjustment due to the mounting screws being loose.
While making the weekly airway examinations with federal mine inspector, we found a rock one break inby spad #47 in the primary escapeway from breaks 16-17. All men were pulled from the mines and the fall was reported. Area has since been cribbed, dangered, and Heinzmann Jacked from breaks 15-18 in the #7 intake entry & the primary escapeway has been re-routed.
At 7:50am an unplanned roof fall was encounterd in the secondary escapeway for the number one section. It began approxiamately 50' inby survey station 5261 and extended 5' into the intersection of survey station 5278. The fall was 25-30' long, 16' wide and appeared to be 5-6' in height. All aproaches were dangered and breakered off. MSHA was notified at 8:00am.
Hoist outage due to the encoder drive mechanism being damaged as a result of lateral movement of the clutched drum.
An unintentional roof fall has occurred on the #1 Entry (face) West Submains 27ft inby spad 6838. The roof fall was approximately 27ft x 18ft x 5-6ft. There were no injuries reported and no equipment was involved.
A roof fall was located on the #2 South Section (#18 Block, #4 Heading). The area of the fall was measured at approximated 16' wide x 20' long x 25' high. The area was bolted with 6' tension and 12' cable bolts. A shuttle car was under the fall area.
A grass/ brush fire occurred on the property after an electrical malfunction occurred on a power pole which allowed a hot spark to fall to the ground and ignite the dry grass. Approximately 15 acres of grass & rubbish were burned. No equipment was involved. The fire was extinguished by fire department crews. There were no employees working and this mine is inactive/ non producing.
Employee had partially drilled a hole using a jackleg drill. A coworker discovered him unconscious/unresponsive, slumped over the drill. It was later determined the employee's clothes were wound up in the 6' drill steel though the cause & nature of the accident & cause of death remain undetermined and under investigation.
Employee had worked an entire day operating a John Deere 750C Dozer along side another experienced dozer operator. At approximately 5:25pm the employee was last seen crossing over a dirt pile. A few minutes later that same employee saw the 750C dozer blade first in a ditch with the tracks still running and employee lying face up behind the machine.
Security guard fell or slipped from the cab of his personal vehicle into a shallow diversion ditch at the base of a berm along a roadway. He was found with his face submerged in approx 12"" of water. The death was drowning & that a contributory condition was marked alcohol intoxication. The case was not determined to be chargeable until December 22, 2014.
Victim was caught between track hoe bucket and haul truck. An excavator was loading material in a haul truck parked in the pit. When the victim exited the truck, he was struck by the bucket of the excavator and pinned against the truck.

Working in Pumping Station. Started to install 18"" pipe with 90 on end, 7' long. Put pipe into position, started to leave it down, when hook on lug was noticed; the chain was under the hook. Chain came around hook, pipe jolted down, sprung back up and then down again, busting the rigging. Employee grabbed the pipe and smashed his thumb.
EE was helping oiler change the cutting edges on a motor grader he had been operating. As EE unscrewed the last nut (edges come bolted together) the cutting edges fell faster than expected, pinching his left pointer finger, resulting in the loss of a small part of finger tip (approx. 1/4"" X 1/2""). All PPE was being worn/used at the time, including gloves.
Employee was assisting a co-worker hook up to a pressure washer trailer. The trailer was not aligned with the hitch on the co-worker's truck. Employee tugged on tongue trailer and had a sharp pain in his left side of rib cage.
Dozer operator was pushing material to the line one feeder at the concentrator IOS pile, when he backed the dozer on uneven ground and tipped the dozer towards the left side. He hit his knee on the inside of the cab, bruising it. He was taken to the ER, and taken off work for until his next rotation.
While pressure washing dozer rotten tree fell of power line which fell to ground approx. 200 yards from where he was working and he received electrical shock to left arm through act of nature.
Dropped fuel nozzle, employee tried to catch it and slipped off machine.

Individual was climbing the ladder on a 785 haul truck missed the top grab bar and fell to the ground.
While tightening a bolt on the drifter of the rock drill, the operator pulled the 24"" box wrench toward himself. He felt a pop in his neck, resulting in severe pain when attempting to move his head.
Truck was exiting the access road to the mine and overturned. Driver had minor bruising to his shoulder.
Operating excavator in hard rock conditions, machine tilted and dropped back onto tracks.
Employee has been off work since 04/15/2014, on 04/29/2014 is when the injury turned from S&A to workers comp. Employee has two bulged discs in his back and is seeking medical attention. We have no incident to relate this injury too, there for this will be degree six.
Injured employee is allegedly claiming a 8 inch rock fell from an area overhead at the primary crusher frame. Striking him in the right shoulder. The primary crusher was not operating during this clean out process.
Employee reached in between a locomotive and a rail mounted supply car to align the couplings and got pinched between the locomotive and a pallet of concrete blocks that were sticking over the end of the supply car. As a result of such, the employee incurred two broken ribs.
Employee was putting drill steels together when a piece of draw rock fell scaring him. He had his hand on the joy stick and hit the thrust causing drill steels to go thru his hand.
EE was unloading a piece of steel (2x6x50) off the forklift & when she pulled the bar off the forks, her fingers got caught between the bar and the toe board. She was evaluated by a Physician and diagnosed with a fracture to her 3rd & 4th finger. She was taken off of work.
The injured employee and a helper were making a belt splice when the helper struck the injured employees index finger on the right hand with a hammer.
Injured was examining the brakes on a manbus when the hood support fell causing a caught between injury to the left thumb. The injury was first reported on 4/29/14 as a first aid injury. On 5/28/14 addition medical treatment was requested and provided, revealing an undiagnosed injury. Surgery was performed on 6/4/14. The first day of lost work was 6/4/14.
Ind. was mudding a wall with B Bond cement when the cement splashed above his rubber gloves and ran down inside the gloves causing it to burn his arms.
While walking from vehicle to mine office, blacked out and fell to the ground.

Employee was roof bolting and had positioned a roof bolt in front of him resting on the bolter wrench holder. As he lowered the bolter boom it came in contact with the roof bolt knocking off the expander shell and it struck him in the mouth. He received a laceration to his lip requiring 2 sutures and chipped 2 teeth.
EE Came in early to load supplies for a belt lay. He was tramming a scoop with his hand on the canopy with finger tips exposed. When the top got lower it mashed his left ring finger between the canopy and the roof. He went to union county hospital and was transferred to Louisville where they pinned his finger that night and released him that next morning. began missing 4-29-14
Employee was cleaning the firebox at the dryer and had his fire boots on. Once cleaning was completed and employee removed his fire protective boots he noticed a blister on bottom of right foot.
Employee was rib pinning on the left side of the miner when a rock fell from the mine roof pinching his right middle finger between the rock and the pinner. He suffered a fracture and 2 sutures from the injury. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
Employee was holding a roof bolt that was being used to drive the pins out of the cat pads on the joy continuous miner when a co-worker who was using a six pound sledge hammer to strike the roof bolt with missed the roof bolt and struck the employees right wrist causing fractures to the wrist and hand
Employee was setting a shield when it pick up the adjacent shield and the pontoon came down on his right foot pinching it. It resulted in a fracture. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
The employee was working in the third floor when he swing his body hit over the eyebrow with something fix and stationary.
On April 30, employee reported shoulder pain after slipping and catching himself dismounting haul truck. On Friday, November 14, employee confirmed plans to undergo surgery to repair torn tendon. He has not missed work or been restricted from normal work activity to date. Surgery to be scheduled for December or January.
Employee was helping carry Jack leg drill. His left leg gave out. Pre-existing condition pins popped in leg.
Employee was moving miner cable and felt pain in his back.
Rock truck operator was getting off truck at approx. 5:00 when he slipped from bottom step and falling to the ground, hitting ground hurting lower back.
Employee was shoveling spillage on 2 East mains belt. While moving a Wheelbarrow, it kicked over and caused the employee to twist her right knee. Became a LTA on 5/12/14.
A grating covers a concrete tank. When employee walked across the grating one section (approx. 3'x5') slid and fell into the tank. He also fell in the tank from 17' on to the concrete below. He fell on to his right side and fractured 7 ribs, his elbow and shoulder.
As employee was getting on the roof bolter a rock from the top fell on his back and continued down striking his right leg above his ankle. (fractured right fibula)
EE was loading belt structure into scoop bucket from supply car at the end of the track when he felt a pain in his lower back.
EE was tasked with removing and replacing the corner edge of the 565 loader bucket; while removing the cutting edge, using the impact to remove the bolts, the mechanic's hand was cut.
The employee was pulling a rock dust hose through a door and around a corner. as the employee pulled the hose around the corner the hose got tight the employee pulled the hose and felt a twinge of pain and then a cool burning below his waistline. The employee did not start to loose time until 06-06-2014.
Employee was lifting cribs when he felt pain in his shoulder.
EE was in the process of setting/leveling unit #1 header. He tripped on a crib block causing him to fall. When he fell his left leg hit another crib block causing a laceration below his left knee. EE continued to work without pain. He seen his family Dr. on 5/1/14 due to pain in his left leg. The laceration had become infected. EE began missing work on 5/2/14
Employee was checking the fuel on a welding machine, which is located on the back of a flatbed pick-up. Employee climbed up the driver's side of the bed, instead of the rear of the bed. Employee slipped on the pick-up's fuel neck, which is located on the bed. Employee tripped and fell to the ground, landing on his right foot. His ankle broke/dislocated upon impact.
While putting bearings on a tail roller he bent over to pick up the bearing and felt pain in stomach. ***Began missing on 5/9/2014***
Grabbed hood and put feet on bumper of Kenworth truck by himself to try to lift hood. As he pulled he felt pain in lower to mid left side of stomach. Taken to ER and CT was done. Showed negative and doctor said he had pulled a muscle.
Employee was checking level of the reclaim reservoir. Walking back to the truck the employee stepped on a rock and fell on his right shoulder. **Employee did not begin losing time until 05/15/2014. (Surgery)******
Employee was loading shot material into rock trucks. He stated he was prying on a rock and the teeth on the loader bucket slipped off the rock causing the endloader to shake him. During the shift he began to experience pain in his right hip. His first day off work was 4/28/14.
Cable was too short for the task, powerclimber rode off cable, basket fell. Laceration on face, head trauma.
Employee was setting 6X6 timbers when he went to put the wedge in the timber and it started falling so he reached out to grab it with his right hand pulling his shoulder. EMPLOYEE HAS BEEN WORKING SINCE DATE OF THE ACCIDENT AND STARTED LOSING TIME ON 6/26/14 DUE TO A PARTIAL TEAR RESULTING IN SURGERY.
Employee slipped on an uneven surface while shoveling debris into a wheel barrel. The employee struck his middle finger on the edge of the wheel barrel as he caught himself. The impact resulted in a fractured middle finger.
Cleaning return roller with shovel and return roller pulled shovel and right arm between conveyor belt and return roller bruising the right arm and wrist.
Unit #10 truck coming from 9 pit Job #7 when truck stalled on him. Started rolling back. Driver jumped from truck.
PICI Contract employee strained his lower back while helping hang the miner cable. He was lifting cable with his left shoulder and slipped on a rock and the cable fell on him.
A Grizzly Rock Separator was being repaired, but it was not properly blocked against hazardous motion. It shifted forward and stuck a miner. The miner received a broken shoulder, arm, finger and head injuries.
EE crossing the section tailpiece, EE slipped and fell on belt. Scrape on forehead.
Employee was assisting MSHA Inspector measure opening of regulator. When she went to hand tape measure back to inspector she was sucked into opening resulting in abrasion and bruising of Rt knee. Employee removed from work on 4/26 by Dr.
Part of a crew replacing the belt on a feed conveyor (C-397). He was working in a manlift. Attempted to push the conveyor belt into the center of the troughing roller. He felt a twinge/pull in his left shoulder. Pushed a second time on the belt, the pain was more severe and he had a hard time moving his left shoulder, i.e. the pain worsens with movement.
While dismounting a loader miner reportedly slipped and fell onto his back.
Employee reported lower back pain after operating a dozer. The medical evaluation identified degenerative conditions in the lumbar spine. Diagnostic results were obtained 5/7/14.
Employee was loading Kennedy overcast panels and a panel slid off the flat car striking his foot. He continued to work but the pain persisted after stepping in a hole at home while splitting fire wood. He went to the doctor on 5-11-14 and was diagnosed with a fractured fibula. EE DID NOT REPORT AS AN 'ACCIDENT', AND EXPECT TO BE TAKEN TO A CLINIC UNTIL THE MORNING OF MONDAY, APRIL 28TH. EMPLOYER IS DENYING CLAIM BASED ON INFORMATION THAT WAS EXPRESSED TO THE GENERAL MANAGER AND FELLOW WORKERS.
Employee was changing bucket teeth on a loader. Employee used a hammer to remove old teeth. Afterwards he noticed a small laceration on his left forearm. Two days later his forearm was sore so he went to the hospital to get x-rayed. A small shard of metal was lodged in his forearm but the doctor couldn't find it and put one stitch in his forearm.
While stacking bags on a pallet employee pulled back. I am filing this form pertaining to a part 50 audit ( citation 9310799 ).
While tramming shuttle car rib rolled off hitting his right arm.
On 4/25/14 the employee reported that he had a strained groin that was suspected to have occurred on 4/23/14 when pulled cable slack from a shuttle car reel. This is being reported as a lost time injury.
IE was replacing top structure on #6 belt when he strained his left shoulder. IE continued working his regular job until 08/21/14 when he had surgery to repair left rotator cuff.

Injured employee was a passenger in a pickup truck of which the driver lost control of the truck just south of the 18 BMP ditch. He drove through the south side BMP berm and landed on the North side of the BMP water ditch.
Employee was operating a 773F haul truck and stated the bed was bouncing up and down when empty, causing him to injure his lower back. Employee did not Communicate to anyone that he was injured until 04/29/2014.

The employee is claiming back pain from opening rail car lid hatches and recently had surgery. The incident was never initially reported to plant personnel as being work related and was being paid for by the employee's personal insurance. The incident is currently under investigation.
Employee stated that he was attempting to lift a timber out of a scoop bucket when he felt a sharp pain in his lower back and left leg.

An employee and injured employee were moving an adjustable top rail to the 30 belt conveyor. As they were descending stairs, EE set his end down to reposition himself. IE was at the top of the stairs and felt the rail move. IE tried to reposition his hand when his finger got pinched between the rail and the top step.
Injured employee was walking from the continuous miner on the section to the auxiliary fan when the ventilation tubing fell striking his head and neck.
The employee was riding on the inby end of a diesel mantrip when it came into contact with another mantrip. The end result of this accident was a cervical strain/sprain and thoracic strain.
While attempting to set a timber the employee was holding the timber. When the cap piece was struck with an axe a piece of roof rock 10-inches X 10-inches X 3-inches fell and struck the employee on the head and right shoulder.
Employee stated that he stepped off of a rail flatcar backwards. As he stepped down to the ground he took one step backwards and twisted his left knee.
While stepping down from the last step of the haul truck she was operating, the operator slipped and fell to the ground breaking a bone in her hip.
Injured employee was struck on the knee by a shuttle car cable. The shuttle car was stuck in the mud on the section and the scoop was being used to attempt to pull it out. The employee was standing behind the shuttle car and as the pump motor was activated the slack was pulled up and the cable struck his knee. Employee did not begin missing work until 04/21/2014.
The employee was walking past a trip on the haulage when he stepped on rock in the walkway spraining his right ankle.
While walking tripped on a rock in the haulage road fell on chest and right rib area, wearing proper PPE, Individual continued to work until 4/22/2014 when he was removed from work due to pain in the right rib area.
Miner removing a reamer bit from the cubex; had his hand on the bit reached up to the controls to release the bit. Pulled the wrong lever and the brakeout tool slid forward, instead of the bit being released.

EE was working on a machine. The cab of the machine came down and caught right hand. The right hand middle finger was fractured and there is a 1.8 cm laceration on hand.
Employee was walking when he slipped and struck his left knee on the ground.
Employee was shoveling material on 04/16/14 when he felt pain in his abdomen and noticed swelling later due to a hernia. His initial nature of medical treatment was Treated & Released. He did not begin missing work until 06/10/14 due to surgery for the hernia.

EE was installing a gear on a motor shaft for a crusher, while tapping the gear with a hammer to align gear with keyway on shaft, a 3 millimeter fragment of the gear chipped off and hit EE in neck. EE received puncture wound to neck.
Employee was cleaning off the top of the LW shearer when a rock fell from the mine roof and struck him on the tip of his right index finger. Resulted in an open fracture and laceration that required sutures to repair.
Employee stated he strained his left shoulder while using a jumper cable for changing out ram car batteries. He was taken off work on 4/18/14.
While getting off John Deere loader that had been parked at the shop in a well lilt area, he began to dismount the loader and put one foot on the top rung and went to step down with the other foot, and lost his grip on the hand rail with his left hand. He swung around, but couldn't hold himself. He fractured his right pelvis. Should return to work in 4-6 weeks, per his doctor.
Working in pumping station. Started to install 18"" pipe with 90 on end 7'long. Put pipe into position started to leave it down when hook on lug was noticed that chain was under the hook. Chain came around hook, pipe jolted down and sprung back up & down again busting rigging. EE gripped pipe and smashed his thumb. Chain fall, beam clamps, chain jacks, shackles
Employee stated that as he was backing the Continuous Miner out of a lift, he was handling the cable and waterline at the same time. When he stepped backward, he stumbled on a rock and fell striking his lower back. Employee continued to work until 8/15/2014 when he started missing work due to this injury.
While striking a roller bearing with a ball-peen hammer, a small piece of roller bearing broke off and struck the employee in the right groin area. He was evalauted by a physician and taken off of work.
Employee was walking to the tailgate to reset a faulted shield during the mining process. When he reached 125 shield a bounce occurred. The bounce threw material toward the shield employee was standing on. The material hit the bounce guard which resulted in employee being knocked over striking his ribs against the shield. This caused a small laceration to his spleen.
IE was removing a rope that had been used to secure a rock prop to the belt structure. In process of removing the rope his right hand came in contact with a troughing idler causing a fracture to right wrist.
4/14/2014 employee was installing a wear plate under a VSI crusher he felt a pop in his right shoulder. Employee was able to continue normal duties, with restrictions. Doctor ordered a MRI, and the employee sustained a rotator cuff tear. July 2,2014, employee had surgery. July 7,2014, doctor would not release him back to work until he saw the employee July 10, 2014,hence the LTI.
Employee was turning to leave the West B drive/take-up area and struck his head against the 2"" water pipe running from the track entry to the drive area jamming his neck. He started losing time for this injury on 9/19/2014.
EE was working as a team lifting a 4"" beam, when the two men lost their grip on the beam, and it fell to the ground, landing on the right boot. Injuring the employee's 4th toe (one over from pinky). He took his boot off, no damage was seen, so he put his boot back on and finished shift. After the shift was over, he decided to go to hospital. His toe is not broken but infected.
Employee was attempting to attach a hot start hose to a dozer and experienced pain in his wrist. The employee was evaluated and received a full return to work release. A follow up visit was conducted and the employee received a release with restrictions for his next scheduled shift on 05/28/2014.
Employee reported soreness to the back and shoulder after operating a haul truck.

On April 13, 2014, employee reported lower back strain after jarring back while operating bulldozer. Physician issued lifting restriction on April 21 that prevents employee from fulfilling normal duties.
Contractor/mechanic was putting covers on a miner when one of the covers slipped smashing his finger on his right hand.
Employee arose from a sitting position while on break and temporarily blacked out causing him to fall forward out of the conductor's shanty and onto the track ballast rock resulting in lacerations to the forehead and chin requiring 1 staple and 3 stitches.
Employee and another employee was shoveling #1 belt when injured employee said his arm was hurting and had a bump on it. I asked what did he do he said that he never done anything to it just shoveling and kept doing it. The next day he still complained and went to the doctor.
The employee was loading tools onto a belt trailer. The trailer fell off of the jack onto his right foot.
While working at the belt head, the employee got his foot stuck in the mud and twisted his knee. He began missing work on 5/13/2014 when he had surgery to repair his knee.
Hanging miner cable and water line, with assistance, in 22D from 1 Right to 2 Right. Left thumb was twisted and bent backwards as employee was raising cable to place in cable hanger. Cable was approximately face level when injury occurred. No problems with mining equipment, training, PPE or compliance issues. Diagnosed with sprain/strain of left thumb.
Pulled bottom roller off of the shield car and one end of the roller came off striking the employee on the right little toe.
Employee was shoveling salt from under the cage paktor belt and experienced a sharp pain in lower back.
Employee was moving belt structure, when he felt a pain in his right arm around his elbow.
Employee was operating a shuttle car and while attempting to turn into a left crosscut he struck his left arm on the rib causing a sprain/strain type injury to his left shoulder.
Filling floor cleaning detergent and it splashed on his left foot. EE did not report it until the following day. I met him at the clinic. They sterilized his foot and recommended to not work until healed because of chance of infection.
The injured said that he was cleaning up tunnel #4. He had finished hosing the tunnel and was dragging the hose with him as he left. He tripped on a rock and fell forward into the superstructure of the conveyor in the tunnel. The injured fell face first into a piece of angle iron on the conveyor, striking his mouth. He broke 8 teeth and his lower jaw.
Mechanic was hosing out a small overflow pond. He slipped and fell. reportedly sliding down the incline of the lined pond with an approximate 40 degree slope 8 feet to the bottom of the empty pond.
Employee was stepping down off feeder up in washer when his foot was caught by a bolt connected to the feeder and twisted his left ankle.

The employee was watching another employee hydroseeding with the hose when the other employee lost his footing & stumbled & employee ran to assist the other employee, fell and rolled down the slope, pulling the muscles in his neck.
Mechanic was standing on the rear bumper area of a Euclid R40C water truck and was connecting air lines to the spray valves. He reached into his pocket to get a wrench and inadvertently stepped back and his foot slipped off the bumper area. He fell to the ground and landed on his right side, causing fractures to his ribs.
Employee was operating a walk-through bolter and installing a roof bolt into the mine roof. As he was handling the bolting supplies he pinched the tip of his right index finger between the bolting supplies. He started missing time on 4-10-14.
Employee was installing cable bolts and when he stepped off the machine, he stepped in a hole in front of the Roof bolter twisting his knee.
Injured employee stated that while walking by the loading machine he tripped and fell on coal spillage causing him to strike his right knee on the loading machine resulting in a bruise.
While re-installing a machine safety guard associated with a primary clay crusher, a maintenance repairman lost his footing while working off a 41"" high elevated surface falling feet first, his back and neck striking nearby surfaces causing injuries.
Employee was splitting stone and the stone broke causing a part of it to strike his right foot on the top of the arch.
Miner was breaking block with splitter. When miner went to pull splitter from block, block fell and pulled miner down, injuring the miner's arm and knee. Miner was sent home by manager the following day due to soreness and then miner sought medical treatment. Miner has not returned to work since.
Employee was getting off of Coal stockpile truck unit number 1146 (HD 325 Komatsu) when he slipped on rough ground. He said it felt like something pulled in his right hip. He went to doctor on 04/09/2014. He worked on 04/10/2014. Went back to the doctor on 04/11/2014 and was off till 04/16/2014. Lumbar strain.
EE stepped in dip in mine floor that was hidden with a coat of rockdust, causing pain to his RT knee. EE continued to work regular job. On 4/18/14, MRI showed a horizontal meniscus tear & degenerative changes in RT knee. With medical information, no traumatic injury and performing normal duties, this will be reported as an Occupational Illness. First day off work is 5/7/14.
Employee was beside track rail with a track jack under the rail, raised six inches, and the track rail fell off the jack, landing on his foot resulting in fracture to the foot.
Lifted a rock to move it out of the way. Hurt back.
Injured employee was accessing the Komastu PC 270 trackhoe. He used his right foot on the crawler step and placed his left foot on top of the track. When he placed weight on his left foot, his left knee extended outward to the left resulting in a strain.
Injured employee was roof bolting on an overcast ramp when a piece of rock fell from the top and struck the 4-hole strap to be installed causing the injured employee to fall into the roof bolting machine resulting in two fractures to the small bones of a vertebrae.

Ee was changing cutting edges and one fell onto the ee's right foot. He was seen by a doctor and diagnosed with three broken toes. Follow-up with foot specialist resulted in restricted duty. Ee put off work when restrictions could not be accommodated.
EE was lifting 2 miner cables & water hose in cable handler when he felt a pull in his abdomen. EE worked through 5-6-14. On 5-7 the EE had a ventral hernia repair done.
Employee slipped on a rock while working with water pumps
Using a pry bar to move track rails when bar slipped individual fell forward, pain in the middle of back, trained on job task, wearing proper safety equipment.
Employee slipped on a flat surface, grabbed loader ladder to break fall, strained right shoulder. No medical treatment sought until 4/21/14, Physical Therapy Prescribed following 4/21/14 medical examination.
Employee was fueling a dozer and he slipped and fell to the ground approximately 9 feet.
A platform fell off of the wood dunnage striking his right foot above the steel toes. Contusion to right foot.
Employee was cleaning coal in the pit on a D10R dozer. At approximately 12:30pm he started experiencing pain in his lower back. He did not report his condition to his supervisor as an on the job injury until April 11, 2014. The injury became recordable on April 17, 2014 when he was issued work restriction by a doctor.
Employee was climbing onto a Cat 777B End Dump when he heard a pop in his right knee. The accident happened on 12/13/2013 but the employee continued to work until 04/07/2014.
Employee was making a mechanical splice on belt, while swinging a 3lb hammer to drive required rivets he strain/sprained his shoulder. PPE was not a factor/ mining equipment was not a factor/ mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.
While walking, employee slipped on a cable and fell at the stationary loader. When he fell, he caught himself with his right hand causing an over extension of his right wrist and a simple fracture to the right hand.
Employee was hanging miner cable and stepped on a small piece of roof rock and felt pain in his left foot. There is no mechanism of injury due to the sole of the work boots being extremely thick. He has had no prior foot pain due to his pre-existing condition of having a cavovarus foot. He was taken off work on April 11, 2014.

Unloading a piece of cold roll steel 4"" by 7' when it shifted catching the left middle finger between the steel and a rail on a pick up truck.
The employee reached into a moving conveyor to attempt to remove an empty bag that was caught in the roller. While attempting to remove the bag, his arm was pulled into the conveyor.
He was a passenger in a mantrip. He lost circulation in his legs due to lack of activity. When he exited the mantrip, he rolled his right ankle due to lack of muscle control. His metatarsal boots were in good condition and there were no irregularities to the mine floor.
Started de-watering holes, finished a row-backed powder truck to start next row. Powder in bulk truck jammed up in bin-EE climbed on top of truck to poke powder (break loose) pole was too long-he decided to lower hand rail to break pole-pole broke causing EE to lose balance-falling to ground. Broke left leg, left shoulder dislocated, bruised left lung.
Employee was moving a cage from a piece of large machinery and his foot was caught between the cage and the machine. He suffered bruising and swelling. Xray shows nothing was broken.
He was installing roof support with a hand held air drill. He was wearing his protective equipment, and trained properly for the job. He examined the roof and confirmed it was sound. A piece of material fell from the roof and struck his left thumb causing a fracture and laceration which required sutures.
Worker was using a JLG450A Man lift while welding on a hopper. He failed to realize that the boom was in a reverse direction. He moved the lift in the wrong direction and was pinned. He had fractured ribs and vertebra.
Employee slipped while walking across the cement pad and fell onto his right shoulder
EE had just loaded a shuttle car w/the continuous miner when a rib rolled & fell onto his leg. On this date, he was transported to a Medical Center ER & was told there was no fracture. On 4-21-14, he kept experiencing pain & soreness & went to a different Medical Center where a 2nd X-ray was performed & was told he had a hairline fracture of tibia. His 1st day missed was 4-22-14.
Employee slipped while walking which caused a fracture to his fibula.
Operator lost engine control of fuel/lube truck on decent of haul road at the end of the shift. Operator veered the truck against a highwall to stop it. Operator struck his knees against the dash and forehead on the windshield. minor bruising incurred
Employee was hanging cable when he was shocked. The employee was brought outside and taken to the hospital for observation. The ER physician wants the employee to take a week off as a precaution.
Injury occurred while pile butt was repairing handrail at the Quarry spring line. He cut the handrail away and lost footing resulting in a fall to the water below, where he struck a float in the water. This resulted in two broken ribs and a punctured lung. Employee will be off duty until he has rehabilitated. Lost time injury.
IE was washing crusher roller area when he attempted to reposition himself; as he was moving, he bumped into the open inspection door causing him to lose his balance and fall into the bearing block.
An Employee was walking on an elevated platform with handrails. The employee opened a spring loaded gate of a fixed ladder opening and stepped in the opening falling approximately 7ft.
Strained himself while lifting beams onto cribs.
Employee tripped and fell on his right side as he stepped from a ladder onto a granite block.

While helping make a belt splice he was moving the knocker his finger was accidentally hit with a hammer by another employee.
EE dropped a long handled scraper into hydrated lime. The hydrated lime splashed into his eye. He was not wearing his safety glasses properly, they were being worn on the tip of his nose.
Employee was clearing a blockage in transfer chute of tunnel screw that was carrying hydrate into a collecting screw. Upon clearing the blockage, hydrate was forced from the access port onto exposed body parts causing lime burns to the head and facial area.
EE was holding a ladder for another ee to the side of the loader forks when a rock fell off of the forks hitting ee in the right pointer finger and right leg.
Employee was ascending a concrete walkway along side of the Primary Crusher conveyor belt when he slipped and fell, striking his left index finger on the steel conveyor railing. Result-laceration requiring 4 stitches.
Lifting a steel cover plate off the side of the Long Wall Shearer felt pain in lower back and left hip, trained on mechanical work and wearing proper safety equipment. Proper lifting procedures may be a factor and foot placement in a muck bottom.
Employee bent over to pick up can of gear oil and felt a sharp pain in his lower back. Employee was treated at a Chiropractor in Charleston.

Employee was operating a continuous miner loading a shuttle car when a piece sand stone came out of the conveyor on the miner and struck him on the right side of his hard hat.
Mechanic was welding a patch on a discharge chute. A piece of hot slag fell on mechanic's coveralls, burned through his jeans, and started a small fire. The mechanic's right calf was burned.
While unloading wooden track ties (for 85 lb rails) from a flat car and stacking them along the rib, the employee felt discomfort in his lower back. No problems with mining machinery, PPE, training, or compliance issues. Employee did not follow proper lifting procedures.
Employee was walking around a Ram Car he had been servicing, when he stepped on a rock on the mine floor and twisted his knee.

Employee was reclaiming some belt framing. He stepped off of a supply trailer onto the ground and twisted his left ankle. This resulted in a sprained left ankle, that did not allow him to return to work.

This injury occurred on 04/01/2014. The employee did NOT begin losing time until 06/19/2014. Employee was using a double-jack to loosen a concave. After his second hit, a piece of metal liner came off hit the employee just above his left knee. Employee worked until surgery date of 06/19/2014.
EE states he was turning a reducer and felt pain in his right shoulder. Medical treatment obtained on 4/2/14 and put on modified duty. The injury was reported late and there were no witnesses. The injured EE was concentrating on the clean up prior to the accident. Apparently a rock from the mine roof located at 4C fell out bounced off the floor then flipped over striking EE.
While coming down a set of stairs the employee says she heard a pop in her knee and felt pain. Initially this claim was ruled as non-work related and the employee did not allege a hazard. Since then the claim was accepted by worker's compensation and we are now reporting the claim.

EE was poking the roaster when he felt sharp neck pain that radiates down bi-lateral upper extremities with pain and numbness. He returned to work, however on 4/28/2014 the pain was continuous and he went back to the clinic for the radiating pain and numbness in his neck. He was taken off work at that time.
Operator strained his shoulder. Believes he did it on the ladder of a 988F Caterpillar loader. Shoulder Rotator Cuff injury. Awaiting MRI to assess damage.
Employee stated that he was installing a rib bolt and as the bolt entered the hole it bent and then the rotation was engaged, which caused the bolt to strike/twist his hand and wrist. This resulted in a contusion and restricted work duties.
Employee was operating the gob side walker shield and kneeling on the pontoon, when he began to push out on the E-bar his right boot became caught between the pontoon and the e-bar. The injury resulted in a contusion. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
Employee accidentally pulled the trigger for the torch while cleaning the tip of it with his glove, burning some of his finger on his left hand.

The ee underwent surgery to right knee on 09/29/2014. On 12/29/2014,company received causation findings stating that an acute on chronic injury meeting the 1% WC causation requirements occurred.
Employee stated a rock fell a short distance while he was on top of the continuous miner repairing a water hose going thru the belly of the machine. The employee continued to work but on 5/5/14, doctors placed him on restricted duty. As of 5/19/14, doctors have him off work.
Employee was filling bulk bags. It was a long step down from the platform he was standing on. When he stepped down, he felt a pinch in his abdomen.
Miner was operating the Hydra-split machine by sliding/pushing a piece of limestone to position it against the teeth guides with his left hand, and as he activated the cut lever his hand slipped over the top of the piece of stone and into the splitter resulting in fracture and laceration of his left index and middle fingers.
The employee had a drill steel stuck in the roof. When he pulled on it to get it out the steel came down on his finger smashing it between the steel and the drill box. Resulting in stitches and a small fracture to his right little finger.

As the employee swung the sledgehammer over his head, he felt a ""pop"" in his left shoulder and released the sledgehammer, which then struck the same shoulder (left). The employee has scheduled an MRI to determine the extent of the damage (an x-ray revealed no damage).

Two men were putting the rub rail on the continuous miner. The rub rail was dropped and struck this man's right foot. The foot was fractured, cut, bruised, and swollen. The cut required 10 stitches.
While using a box cutter knife to cut the long ends of plastic wire ties, the knife slipped resulting in a 2 cm laceration on the top of the victim's left hand. The wound was closed by a physician with two stitches.

I went to the tool room to get a new tip and nozzle for my welder, and I got tangled up in the torch hoses, and fell out of the tool room on to the ground. After getting up something popped in my back and that is when I tried to walk the pain out but it didn't help and I decided to go to the Emergency Room to get checked out.
Employee was carrying buckets of stone for stemming holes on a stone shot when a lower back strain occurred.
On 4/16 employee dismounted forklift without facing the forklift or using three points of contact. Employee lost balance and twisted right knee when his boot caught the lip of the step. He said he did not need medical treatment and was retrained on proper mounting/dismounting procedure. On 5/13 he requested medical treatment for pain in right knee.
Beltrami was tightening a spring break on #85 tractor with a wrench and pulled his right shoulder.
Employee tried to grease a bearing while the machine was still running. He has been trained on how to operate this piece of equipment and knew that it was NECESSARY to turn the machine OFF BEFORE any maintenance was to be done. As a result, he slipped and broke his finger.
While loosening a nut the EE felt a small pull in his stomach area. The employee was sent to a medical facility and was diagnosed with a hiatal hernia.
Dredge operator was releasing the brake on the positioning winch, as the pressure was transferred to the winch handle he lost his grip and the winch handle came back around striking his hand. The supervisor took employee to the clinic and treated for swelling. On 04/16/14 employee was examined and it was determined that he has broken a bone in his index finger on right hand.
Employee was lifting the lid on a mechanics tool box, when he felt a pain in his lower back.
Employee fell at work bruising his right knee on 4/14/14. On 5/29/14 employee went to the doctor to find that cellulites had set in his right leg. Employee was prescribed antibiotics to treat the illness.

Employee was stringing out welding leads and placed the welding leads over her left shoulder. Employee began to walk with the leads and lost her footing causing a fall to grade. As the result of the fall the employee's left shoulder sustained a small chip from cartilage from left shoulder socket.

Employee was cleaning at tail section of conveyor belt k and his left glove got caught on a trough idler and pulled his left arm in through the idler to his forearm. the left arm was squeezed between the idler and the conveyor skirt frame. He received abrasions to the left forearm.
Employee placed a 4x4 underneath a portable pump to be able to pick the pump up with a forklift. He signaled the operator to raise the forks. The 4x4 snapped pinching the end of his little finger causing a small fracture.
While dismounting a piece of equipment an employee twisted his ankle. He was released to restricted duty.
EE was cleaning up after installing new conduit, when he attempted to lift the threader he heard a loud pop in his left arm and a sharp pain in his left bicep. THe EE reported to the safety office where he was taken for a medical evaluation and diagnosed with a full bicep tear.
The employee stepped out of the dumping end of a shuttle car, causing a minor tear to his Medial Meniscus. He was able to perform his regular work duties until his surgery. He started missing work on 4/23/2014.
Installing clam bucket on excavator, trying to hook up hydraulic line on 375L excavator CLCN05 hydraulic line to bucket, popping in right elbow. It happened on 4-7-14 but still working full time, was not stop time loss until his surgery on 7-1-14.
Employee stumbled while walking and smashed his little and ring fingers of his left hand.
EE was behind the mesh in 2CA South and had put in the rib bolt using 3162. EE observed some loose material & moved under the mesh to hand scale. It seemed to be a small amount to remove, but a large rock came loose and landed in the basket. The rock struck EE in his left shoulder.
Injured was operating a twin head bolter. As he swung the boom over, a piece of mine roof hit the boom lever causing it to continue to swing. The boom hit his leg resulting in a fracture.
Employee was building a stopping, and while attempting to place a stopping block on the stopping wall while working from a ladder, the ladder kicked out and the employee fell to the ground and the stopping block struck him in the left side of his head. The employee received 10 sutures.
Miner was hand loading rocks into the bucket of a front end loader. As the miner was turning to put the rock in the bucket, he caught his hand in between the loader bucket and the rock which was in his hand. This caused his finger to get injured and cut. Weather conditions were not a factor in this accident.
Struck ribs against the operators compartment of the shuttle car, when shuttle car lost power, broke 2 ribs, and bruised spleen.
Employee was winding up a hose in the Pellet Plant. The employee went to realign the hose and felt a pop in his left knee while planting his foot on the floor.

Injured party is not an employee of the mine. Mine personnel were assisting a vendor unload metal culvert pipe that he was delivering to the mine. One pipe had been removed with a forklift and placed on the ground. He then walked around the trailer to the side opposite the forklift to secure a strap when a second pipe rolled off the struck him in the leg fracturing the leg.

A production truck driver reported of being jostled while his truck was being loaded, causing back pain. He was evaluated by a physician and given Rx narcotics & muscle relaxant. He was released with restrictions, but unable to report for his 4/2 shift due taking the medications. He returned light duty for his 4/3 shift, was scheduled off 4/4-4/7 & released full duty as of 4/8.
Employee was cleaning and lifted a large flat, causing strain in back.
Employee was removing lockout locks from a column cell when he accidentally got acid in his boots, he didn't notice it at first until later he realized that his foot was burning and he was treated for burns on his foot

Driller failed to remove deck bushing from upper sub per procedure and while trying to thread the 2nd section of steel the top of the carousel hit the safety wrench, breaking the chain which allowed the wrench to fall, glancing off the helper's hardhat the as he was visually checking to ensure that the steel was threading properly positioned outside of the cab on the drill deck.
Miner was hanging a power cord and while twisting his body felt a twinge in his back. Finished hanging cord and reported to supervisor. Supervisor offered to take him to medical facility and miner refused. Supervisor then sent miner home to rest his back the rest of the day. Next morning miner called and said he should seek medical attention. He was then taken for treatment.
Employee was moving a metal framed cart, when his right pinkie finger was crushed between it and a stationary object.
The injured was cutting 6"" dragline cable with a cutting torch into sections to be loaded in a truck. When the cut was completed the cable sprung apart at the cut, slapping him in his right leg injuring his knee.
Miner was lifting half of a 150 lb. deck grate and suffered a hernia on right side
Employee exited his mucker and in the process rolled his right ankle resulting in a sprain. The employee was placed on restricted duty.
Employee was operating a Kubota on the travel road. The front tire caught a notch in the rib due to the employee glancing over in a x-cut while driving and the Kubota turned over on its side. The employee struck head on the roll over protection bar. Restricted Duty.
Injured was inspecting for cracks in the short rake section of the #2 thickener tank rake arm. He used a retractable lifeline and full body harness to descend a fiberglass extension ladder to access the center of the weir to do the inspection. While on the ladder he lost footing and fell striking adjacent steel structure resulting in contusions to his lower leg and a concussion.
Employee injured his left hand while doing repairs to a sledgehammer in the tool room. Employee was hammering a wedge into the top of sledgehammer to tighten the handle to the head, the sledgehammer was clamped in a vice, and he hit the base of his left thumb with a glancing blow while trying to drive the wedge in with a hammer. No broken bones, released for modified duty.
After dismounting a service truck, the employee turned in motion with his feet on the cement floor rolling the right ankle.
He was using a jackhammer all day during 9 hr. shift. Next morn. he woke to pain in lower back. Pain increased over the week so he reported it. Doctor diagnosed lower back strain May 5 & put on modified duty. Currently still on modified duty until May 28-no lifting over 40 lb.
The employee fell five feet from scaffold during erection. The deceleration device on his personal fall protection was deployed.

When pressure was applied to this bar the supporting sling came out of the air winch hook and the load fell on the workers foot.

Employee was reaching in the back of the semi-trailer, the wind was blowing and the door blew shut on employee squishing him between the trailer frame and door. He complained of upper left chest pains.
Hosing out a press slipped and fell. Tried to break his fall and injured his left arm. Originally on Temporary Job Transfer until 5/5/14 when his injury of 4/24/14 became reportable.
Employee's were tripping out PVC when the under reamer slipped. Hitting the employee in that back of the arm. First report was employee hyperextended rt elbow.
While being tied off onto rail fall protection system, as he stepped across between 2 railcars the supplemental restraint locked and pulled him off balance. He fell slightly backwards between the cars, approximately 3 feet. He alone stepped back onto the railcar and reported incident.

Tension on V Belts was being adjusted when employee slipped, to prevent falling he grabbed V Belts. The V Belts rolled backwards pinching his finger between the sheave and V Belts.
Employee was working in the clinker basement hooking up several ladders to the hook of the Model 308 Grove Crane. As he was securing the ladders with a rope, he noticed that the rope was tangled in some other equipment. The spotter misinterpreted his movements with his hand as the signal to raise the load, which cause his RT hand to be pinched under the cable.
While preparing to install a belt scale, he started having back pain that progressively worsened. Unidentifiable as to what caused injury, possible bending or twisting.

The employee was inspecting a 2yd LHD which was parked in the fueling area of the 3700 ramp. There were two 10""x10""x10' timbers leaning against the rib beside the LHD. The timber in the rear fell and bounced off the rib or other timber and hit the employee in the right side of the head as he was leaned over inspecting the LHD.

Employee arrived at work and went to his locker to retrieve his boots. As he bent over to put his boots on, he felt a pain in his lower back.
Employee was stripping a wire for the turn signal on truck #83 at the truck maintenance garage. He was using a razor blade. The blade slipped and cut a finger on his left hand.
The employee was closing #27 press filter feed valve using a hammer wrench. When the employee struck the valve on about the 4th time he felt a pain in his right elbow. The employee stopped working and was taken to the company doctor for examination. The employee was put on restricted work after returning from the doctor.
EE was barring/cleaning material off of interior of lime kiln walls - informed Supervisor the following day that he was suffering from muscle weakness and spasms in his shoulder-primarily on left side. Phys. has placed him on lifting restrictions with Rx. for muscle relaxers/anti-inflammatory meds. Follow-up appt. sched. for 4/29.

Employee was using a rock chiseler and stepped down from chiseling the rock and he stepped down on something that made him twist his leg.

As he attempted to exit the drill, the employee slipped on the top step of the exit steps; (stairwell had a total of 4 steps) and fell, landing on his back. Ice had accumulated on the top step from the wash water the employee was using to clean the drill.
Employee was driving front end loader. Loader ran over pot hole which caused the employee to bounce in his seat, jarring his back causing pain. The original event occurred on April 14, 2014. Employee did not seek medical treatment at the time. Medical treatment did not occur until April 29, 2014 at which time it became recordable.
Employee was cleaning up material using a skid steer loader. A strong gust of wind kicked up dust in the area around him. Material got behind his safety glasses and into his eye causing irritation. He requested medical attention on 4/15/14.
Employee used a torch to cut bolt off when slag built up and went under safety glasses into eye
Employee was working on repairing a conveyer belt underneath a hopper at face level and while cutting the belt towards himself the cutting knife came toward his face and cut his lower lip and chin. Laceration needed stitches. Coworker was working with him on the other side of the hopper.
While employee was bolting #9 right a cable bolt that he was going to install that was laying on the bottom got hung up and he pulled the bolt he felt something pop in his left shoulder.
Employee was opening the freight elevator door (vertical bi-parting elevator hoist way door-requires manual operation) when his right hand was caught between the top of the door and the elevator. Employee sustained a non-displaced right proximal phalanx fracture of the pinky finger and a 2mm laceration (glued) of the middle finger.

Employee was placing the lid back on a feeder and when the lid dropped into position, it pinched the side of his left index finger.
The employee was using a grinder to perform work when the grinder ""jumped"" and caught his left index finger, pulling it between the guard and cutoff wheel. He was evaluated by a physician and four sutures were used to close the wound. He was released back to work with restrictions to keep the wound clean and dry.
Was moving hose and stepped up and had pain in left foot.
Running a half inch Drill Motor the bit caught and the drill twisted around and fractured the employees right hand,
Employee was helping move cable to set up the shovel and while lifting the cable, felt his right knee pop. He recently had a non-occupational right knee surgery earlier this year. He was given restrictions and physical therapy. Did not miss work.
While taking off a pressurized line, the line blew hot water and carbon on to employees arms.
Operator was opening a discharge valve, stepped off of the pump base and on uneven ground and slipped. Felt pain in their right elbow and right ankle. They were taken to emergency room for evaluation and returned to work with restrictions.
Employee was retrieving an SCBA bottle from a storage shelf and hurt his back while lowering the bottle to the ground.
Employee moving a 55 gallon drum strained his back.
Employee had gotten off of forklift to straighten a bulk bag. Employee stepped on partial pallet (uneven surface) and pulled on bag position & force applied caused strain in employees back. Employee felt fine until next day(s) Sat/Sunday & reported injury Monday 4/14/14.
EMPLOYEE WAS REPLACING A RETAINING BOLT ON A HAUL UNIT THAT HOLDS THE CAB TO THE FRAME. IN THE PROCESS, HE FRACTURED HIS LEFT INDEX FINGER.
Haul Truck driver was parked at tailings area waiting to be loaded with material. Loader operator did not see the haul truck and backed into the side of the haul truck. The haul truck driver sustained whiplash from resulting collision.
Employee was loosening a bolt with a crescent wrench on discharge boot on CIL #1. While loosening bolt employee felt a pop in his lower back which turned into a sharp pain that went through his lower back, hips and legs. Employee was taken to the Clinic and put on restricted duty.
A maintenance mechanic was attempting to move a small vertical man lift when he felt a pull in his lower lift side.
Miner is a Mill operator and was operating a C4S Packing machine. Miner was palletizing 50 lb. bags of C4S material when he felt a sharp pain suddenly in his left forearm area. Miner was taken for medical evaluation, and placed on lifting restrictions until a scheduled MRI X-ray can be conducted. Miner is working w/restrictions.
Maintenance employee was crawling from underneath the platform conveyor on the south end of the jug line and the guard fell onto the maintenance employees lower back.
Employee was dismounting 990 model loader, when his foot slipped on the ladder causing him to lose his balance and fall. He landed on his right wrist and sustained a fracture.
Operator was climbing downstairs and slipped. When he slipped he reached out with his left arm to catch his balance resulting in a strain to the shoulder muscle.
Two employees were working on a bearing installation and while tapping the bearing on, a small piece of metal came off and went into the individual's top right forearm area.
EE WAS EXITING CAT 216B SKID LOADER. HE STEPPED OUT OF MACHINE TO GROUND ON RIGHT LEG. BEFORE LEFT LEG REACHED GROUND HIS RIGHT KNEE CAP AREA POPPED CAUSING PAIN & FELL TO GROUND. DID NOT SEEK MEDICAL ATTENTION UNTIL FOLLOWING MORNING WHERE DOCTOR PRESCRIBED MEDICATION AND KNEE BRACE WITH LIGHT DUTY RESTRICTIONS FOR 5 DAYS.
Employee was off loading tires from a trailer in the cross dock area. He pulled a tire from its lacing. It swung around and struck him in the elbow.
Two employees were bagging material into totes. The forklift operator raised the forks while the other employee's hands were close by, pinching one finger between a fork and a chute. The employee was treated at a nearby hospital. The employee's wound was cleaned and dressed, and he was given prescription medication.
The employee was leaving the cab of his utility pickup and he stepped on an uneven surface. The employee sprained his ankle. The employee was taken to a physician for observation and he was diagnosed with an ankle sprain. He received a release to return to work with restrictions.
Working on tail pulley conveyor and bumped elbow causing it to swell up.
Employee was descending the ladder of a front end loader when he misjudged the last step. The employee missed the last step and foot went all the way to the ground unexpectedly. The employee strained his shoulder while trying to hold onto the ladder.
EE was working on Unit 2 repair when a casting fell off where the ee had his left hand placed resulting in getting his left middle finger pinched/laceration. He was evaluated by a Physician where he had the laceration repaired with sutures. He was released to return to work with restrictions.
Injured was dismounting a Cat 972 loader when his hands slipped from the ladder causing him to fall to the ground.
EE was pushing a load with his right leg and felt something pop in his lower back.
Pinner operator was putting up a roof bolt. His hand slipped from chuck and got caught between chuck & head of bolt. Hand was cut between thumb and first finger.
Employee was stepping on the push arm of the 9233 dozer when his foot slipped off causing pain in left shoulder. It was dark and muddy. The slip caused a shoulder strain
Employee was assisting with torquing the rear end of a truck and his ankle gave out. Did not step on anything, shifted his weight and heard ankle pop.
Employee was changing a valve at silos and strained his lower back.
Employee was unplugging the DTV chute with a bar. The tire was stuck so he applied his body weight. When the bar slipped his right hand was in-between the bar and a handrail.
Miner was tramming scoop along #5 entry (travelway) of Black Mains approx. 1800 feet outby the section. A brow of rock popped free of the rib and struck a 5""x7"" post that was 8 feet in length, causing it to fall into the kitchen of the scoop, striking the miner on the left side of his head.
Employee was working off a 4' ladder tightening & replacing a hydraulic line with a 1"" 7/16 wrench. Right hand was being used to tighten the line while left hand was being used to support position. Employee states that as he was tightening the line he felt a twitch in left shoulder & heard a pop, he then climbed down ladder & realized his L shoulder was dislocated.
Employee backed his truck up to a light plant to move it.(LP21) To hitch up the trailer, he physically moved the trailer tongue to the truck. He felt pain in his back and legs. The physician said he has Sciatica and swelling at the junction of S5-L1 in his back. He prescribed steroids, muscle relaxant, and pain medication, and was released back to work on light duty.
Employee stepped down off the steps of the lube truck, he did not notice a frozen chunk under his right foot, when applying full weight on his right foot, the frozen chunk rolled causing his ankle to roll also. A quick look around the landing area before descending the steps would have identified the frozen chunk and could have prevented the accident.
Employee was putting press in service. Valve was stiff and felt pain in back.
Employee, along with three other workers, was taking rollers off of a belt line. One of the rollers slipped and fell back into the rollers channel frame. Employee's thumb was in the channel when the roller fell. Employee received 4 sutures due to the laceration on right thumb.
Employee was driving a van, dropping off haul truck drivers during their first break. He parked the van in the ditch at the ready line. He got the wheel chocks from behind the seat and proceeded to exit the van. As he stepped out, his left foot came down on a rock that was laying in the ditch. His foot rolled off the rock sideways twisting his ankle resulting in a sprain.
EE was inside a cargo container holding plywood in place as coworker was attempting to attach it from the outside, using a power tool to pre-drill the screw holes. Drill bit came through the wood and into the palm of EE's hand, creating puncture wound. Work stopped immediately to dress wound on site before EE was transported to ER for bandaging & antibiotics. Minimal impairment.
While hooking up a chain to the continuous miner (CM) lost footing and right leg slipped under the bumper as the CM was let down causing the bottom part of right leg to get caught between the bumper and ground. Was transported to the ER, x-rayed and released to duty with bruising injury, was re-examined on 4/14/2014 and his right ankle showed a fracture.
Employee was shoveling material next to a belt when he felt a sharp pain in his back.
Miner was performing routine maintenance on a conveyor to remove gear reducer. Miner sprained right ring finger while doing so.
He was jackhammering a concrete block. He had one leg behind the other to get good leverage on the jackhammer. When he stopped and stepped back and stood up straight, his hip felt like it got stuck and felt a sharp pain in his right hip and could not walk.
Employee was pushing a broom, cleaning up coal from around the coal unloading area. She stated she stepped on a piece of coal which caused her left knee to twist. She denied medical treatment and working through the rest of her shift without any issues (incident happened at 9:00 a.m. employee's shift ended at 3:30 p.m.)
Employee strained shoulder while trying to press new return roller into Hot Belt Conveyer. EE was applying pressure to roller, afterwards felt a strain in shoulder that worsened-saw phys. on 4/4 and placed on light duty until follow-up.
