The Dry Ridge Elevator was inoperable for an extended period of time, due to a blown ""O"" ring on the rop gripper pump. A new pump was installed.

An unplanned fall of the mine roof above the anchorage horizon occurred at 12 block, #2 entry on the 8 Right longwall section. This fall was immediately reported to MSHA.
Construction crew was installing steel in the 50E portal and smoke was noticed. The mine was evacuated. When it was determined that it may be immediately reportable, MSHA was notified via phone by calling 1-800-746-1553.
The mine lost power from what was later discovered to be a downed powerline. The cause of the damage is unclear but believed to be a lightning strike or the failure of an insulator which allowed the powerline to burn through the pole. The top of the power pole could not immediately be reached due to the electrical hazard and smoldered for over 30 minutes after discovery.
An unplanned roof fall occurred on 6/27/2014 on the #3 section in the #3 entry at the #17 block at spad #D634. The fall covered up one of our MRS (Mobile Roof Support) units but did not damage the unit.
The primary hoist (#3) went down at 6:15 due to a mechanical failure. All personnel were accounted for. MSHA was notified at 6:38. Mine personnel found that air had leaked out of an accumulator through a leaking pet cock. This caused the oil in the tank to drain and cavitate the pumps. The problem was corrected and the hoist was up at 8:55am
The primary hoist(#3)went down sometime around 1:00am due to an unknown mechanical failure. All personnel were accounted for and the secondary hoist (Galena) was still fully operational. Mine personnel found that the breaker on a 110v inverter had tripped and caused the outage. everything was reset and the hoist resumed normal operation at 2:00am. MSHA was notified at 1:20am.
Employee was traveling on pre-heat elevator when the elevator became stuck between the 3rd and 4th floor. Contractors mechanics were called to troubleshoot the elevator. They brought elevator down releasing the employee. There are elevator entrapment procedures inside the elevator that instructed the employee to contact management. The employee exited the elevator at 2:30 PM.

The Crabapple Hoist car was taken out of service due to a electrical short in the cable that runs from the battery box to the breaker box. No injury as a result of this incident
An unintentional roof fall occurred on 2 North 1 Left Return, Crosscut 52 Entry 2. The roof fall measured approximately 30' in length, 17' in width, and 6' in thickness. A 103(k) Order (9033537) was issued June 24, 2014, by MSHA Inspector.
An ignition occurred on the longwall headgate drum as the shearer was advancing to the tailgate at shield #176. The headgate operator observed a blue streak around the turning drum. He stopped the shearer and grabbed the wash down hose and applied water to the headgate drum estimated time of flames was 10 seconds.
A roof fall occurred in the 5th Right panel off the 3rd SouthEast Submain at XC 26 entry #3. The fall measured approximately 50 feet in length x 20 feet wide x 6 feet in thickness. The area was supported with 5 foot fully grouted roof bolts.
At approximately 4:00 pm a loaded drill blast hole was noticed emitting orange smoke. The lead blaster was the person that witnessed the blast hole smoking and made the decision to evacuate the pit. Pit operations was ceased and blockers were placed in position closing access to the pit.
Excavator 20 feet from a 20-foot face of soil pulling material down. Moisture content of wet soil caused soil to fall in a clod instead of what we generally have observed streaming down loosely. 1 clod rolled up track of excavator, breaking windshield, & pushing it to our operator, not injured except for minor reported scratch on his chest. Windshield broken, ROPS good per CAT.
An electrical breaker tripped on the Koepe hoist and was not properly set causing the hoist to be down. Miners Underground evacuated the mine. The issue was diagnosed and corrected miners returned underground. There were no injuries or other issues.
An unplanned roof fall occurred in the 2nd North West Mains, Spad # 815. The fall was appx. 40' Long 4' Thick. The fall is dangered off at all approaches.
Rock fall was discovered on 9 track Brk 15 #5 Entry. Fall measured approximately 20'W X 50-60'L X 4' T.

A roof fall occurred in the #4 entry at spad 6869. The roof fall will not be cleaned up. All approaches have been cribbed, timbered, and dangered off.
A roof fall took place on the east side of 3 Yard North conveyor belt between 18 Left and 19 Left roadway. The fall hit the conveyor and took down three channels. Part of the fall rolled and blocked the roadway on west side of the conveyor belt. The area was cleaned up and repaired and the roadway re-opened.
Their was an unplanned roof fall on 1 A Road between X 11 - X 12
An unintentional roof fall occurred at the #4 entry #30 cx Cedar Grove mail line #1 belt. The fall was 20 ft long, 19 ft wide and 6 to 8 ft high. A plan was submitted and approved to clean up the fall.
Fire was noticed on conveyor belt, local fire department notified and extinguished approximately 40 min later.
Electrical contactors stuck rendering hoist inoperable. Mine evacuation started at 10:15 a.m. and was completed at 10:30 a.m. The contacts were replaced and the hoist tested. Miners returned underground at 12:20 p.m.
Fall was discovered in the 3rd North East spad (45+00) XC# 57 Entry # 7. Fall approx 35'in length, 19'in width, and 6' in height. High Voltage cable was under the fall but never knocked the power.
The east skip of the #3 production hoist was positioned at the collar and un-clutched. The west skip was then positioned at the collar and brakes set. The electrical power was disconnected. The west skip began to slip in the brakes and moved down the shaft, going to the bottom. The hoist rope broke and both the hoist rope and skip landed at the shaft bottom.
An unplanned roof fall occurred at x-cut #21 on old 6F track. The fall was danger and timber off. The fall was 5 to 6 foot high, 19 foot wide and 20 foot long. The fall will not be cleaned up at this time.
An electrical contactor wire broke and the starter stuck. Mine evacuation started at 5:15 a.m. and was completed at 5:40 a.m. The contactor was repaired and the hoist tested. Miners returned underground at 7:45 a.m.
A roof fall was reported in the North Mains 2, #3 entry, 1 break outby spad 5182.
Working to change the seal on an oil leak on the screen deck. Cut a hole with torch on Seco deck. Used a come-a-long to take the counter balance off. A spark must have gone into bottom of screen deck & smoldered. Flared up at 9:30.
An unintentional roof fall occurred along the #2 belt line in the #7 Primary escapeway entry. The fall was approximately 14' wide by 60' long and 24"" thick. The fall was 6900' feet from the active face.

Hoist lost power to PLC at 12:20 pm. Found shorted horn at collar. Removed short circuited horn from operation. Evacuation started at 12:35 am, cancelled at 1:20 am when hoist returned to normal operations.
The BMX Portal Hoist/Cage was out of service due to lilly over travel fault that couldn't be reset within 30 minutes. There were no individuals injured or entrapped due to this occurrence.
#3 hoist down - ground drive fault. Reset ground breaker fixed at 5:30 am. Evacuation started at 4:45 am, cancelled 5:22 am.
Wood chips were stored in the back of the plant across from the bone yard that were going to be used for reclamation. The wood chip pile spontaneously caught on fire. It was completely extinguished by 8:30 PM.
In process of making weekly exam; Mine examiner discovered a roof fall at 5 wall of the number 1 entry of 3 east mains. The roof fall was a failure in the intersection and partial failure of 1 to 2 cross cut. There were no persons injured or equipment damaged as a result of this fall.
An unplanned roof fall occurred at #2 Entry Spad #152. The fall was apprx. 22' X 22' X 10'. The fall has been dangered off at all approaches. 6ft fully grouted bolts was used with 10' cable bolts.
Fall was discovered 3A belt Break 14 at #6 Heading. Fall measured approximately 20' wide X 60'-80' long X 6'-8' thick.
Employee was walking air courses and found the roof fall. We called the MSHA hotline at 9:20 A.M. We then cribbed and dangered the area off. Air was flowing over the fall, we pulled our crews outby to start corrective actions.
Drive motor coupling bearings on #1 Hoist failed. Miners were evacuated at 10:55 p.m. and was completed at 11:04 p.m. The bearings were replaced & the hoist was restored to service on 6/19/14 at 3:00 p.m. after being tested.
Roof Fall @ 8 BLK on Submains section MMU-007-0 in 4-5 to 6-7 entries in the face. Approximate size of fall is 100' in length x 16-17' in width, and 6'-12' high. Area was bolted with 6', 3/4"" tension rebar bolts. Test holes, a minimum of 8', were drilled every 20'. The cover above the fall is approximately 590'.
The hoist was inoperable due to electrical problems.
Around 6:30 pm a fire was discovered at the St. Leo substation that supplies power to St. Leo fan. It is believed an insulator was penetrated, or failed, causing a short circuit and throwing transformer oil, with the oil catching fire and burning poles and the transformer. Fire extinguished around 7:30 pm. No problem with training, PPE, mining equipment, or compliance issues.
An unintentional roof fall occurred on Unit 4 (MMU 008), Crosscut 13 Entry 6-7. The roof fall measured approximately 50' in length, 18' in width, and 8-10' in thickness. A 103(k) Order (9033214) was issued June 15, 2014, by MSHA Inspector.
Roof fall at sp #360 first right room off of 1st left submains in the return in old works. The fall was 5' high, 17' wide, 60' long. Fall will not be cleaned up. The fall was dangered off and supported as per roof control plan. Fall was 6000 ft from active section.
Roof fall was reported outby on Unit #2 Section. Fall occurred approximately two breaks outby the tailpiece in the intake.
An unplanned roof fall occurred in the Wyoming #2 mine in the #5 entry, approximately 40' outby spad #4009 that measured approximately 50' long X 19' wide X 6.5' high. No injuries were reported. The area was developed on 6/2/14.
Clinker exited kiln, fans were not shut off, and dust exited the dust collector @ clinker silo #22
Low-hazard tailing impoundment cell gave way and released water and sediment into adjacent unoccupied and wooded area. High water level in cell #2 and high winds were contributing factors. A tailings ""delta"" of sediment was created in area of release and some trees/brush in the area were impacted. There were no injuries associated with this impoundment release.
Damage to the control power in the ore pocket prevented the mine from running the #1 hoist. The mine still maintained two means of egress with the #2 and #3 hoists.
Pleasant Grove Elevator broke down due to the control cable being pulled out of the junction box.
An unplanned roof fall was discovered in the Main East Return @ 13-26 crosscut. Dimensions of the fall are 16 feet wide X 50 feet long X 12 feet high. No injuries or entrapments were caused by the fall. Ventilation was not affected.
At approximately 6:15pm the Meikle MaryAnn tripped out due to an electrical issue. At the time of the incident 2 miners were on the MaryAnn. At ~7:30pm power was restored to the MaryAnn and the miners were safely hoisted to the collar.
Unplanned roof fall above the anchorage zone B-Mains xc6 between the number 2 entry and number 3 entry. The roof fall has not compromised the alternate escapeway or the primary escapeway. Primary roof support 7'point anchor bolts with resin. Breaker rows of crib will be installed on both sides of the roof fall.
Tailpiece bearing went bad and caught on fire and was extinguished.
Roof Fall was discovered out by the working face of number one entry approximately 550' to 600' from the section The area was bolted according to roof control plan with four foot resin bolts. The area is in return air way doesn't block ventilation or prevent travel of the air course. All sides of the intersection have been cribbed off to prevent travel into the area.
Found an unintentional fall on the #3 Belt line (#3 Entry) at break # 20 on the #5 PM Panel.
A roof fall occurred on the West Sub-Mains Section (#5 Heading, #44 Block in intersection). The fall area was approximately 12'-14' high and 20'-25' long. Prior to the fall, the area was bolted with 8'-12 combination bolts and spotting 20' combination bolts in the intersection. Going around gas well block and had over 900' of cover.
Power dropped out due to electrical storm, NYSEG fixed, power restored. Hoists are operational.
The elevator at the #9 portal went out of service at 8:15PM. There was no one on the elevator, no one trapped. The incident was caused from a blown fuse in the field circuit. Schindler elevator service was called and the elevator was back in service at 11:15PM.
An unintentional roof fall occurred in the #1 Belt entry of 1 L section at 41 crosscut. The fall measured approx. 60ft long and 16ft high by 16ft wide. No injuries as a result of this event.
Service hoist did not malfunction. Crew was unloading supplies and was not able to jog the hoist to the bottom. Hoist was approximately 3 feet from the bottom. All employees were evacuated until hoist was fully operational, which was approximately 4:32am on 6/7/14. Hoist was inoperable at approximately 2:45am on 6/7/14.
A rock fall was discovered in the 1st N.W. Mains between spad #s (68+49&69+39), XC'S 86&87 in entry # 3. This fall was blocking our secondary escapeway. Approx Length 40', Width 18', Height 7'.
On June 6, 2014 a dragline slid into the water filled pit. No injury or illness was suffered, and no lost work day or restricted days were incurred. The North American Coal Corporation is and was contractually obligated to and is and was legally responsible for operating and maintaining dragline with its own employees.
Employee was hauling stone from a stock pile. A semi-truck & trailer was in the area being loaded by another operator. EE said he was distracted by other activity going on in the staging area and inadvertently veered into the front end of the truck. No one was injured, but there was damage to the truck.
The proximity switch for the #2 hoist failed at 9:45 a.m. Miners were evacuated at 9:50 a.m. The switch was replaced and the hoist was tested. Miners returned underground at 11:20 a.m.
The #1 elevator was removed from service because it would not land properly at the bottom landing. The brushes of the generator regulator were not properly seated. The brushes were seated and the car was tested. The car was put back into service at 12:00.
A roof fall occurred above the anchor point on the old mainline track outby the Whittaker portal at 11 crossover. The fall occurred in a 3 way intersection. It was 20ft long by 16ft wide and varied from 6 to 8 feet height. It didn't involve any compliance issues. No equipment involved. No other 50.20 criteria applies.
An unplanned roof fall occurred in the #3 Entry of 1L just inby #45 x-cut. The fall measured approx. 12' wide x 6' wide x 18' long. There was no equipment damaged or injury as a result of this incident.
Haul trucks were entering heading on 47E and exiting on 50E. Trucks were traveling on 70S to get to 47E and an empty truck did not turn on 47E and continued on 70S and was struck by a loaded truck leaving the face at the intersection of 70S and 50E.
The Dry Ridge Elevator was out of service for an extended period of time. This was due to a bad analog input card. There was no injury as a result of this incident.
Roof fall dated 6/3/2014 was located in the #6 entry of the mains in by spad #530. The fall was approximately 40'L x 18'W x 10'H.
An unintentional roof fall occurred in #7 entry at spad #2228, 23 x-cut 2nd NW Mains, 2320' from working face. 19' wide x 19' long x 8' high. Found by examiner during weekly exam. Fall has been breakered off, fall will not be cleaned up.
Roof fall dated 6/2/2014 was located in the #9 entry of the Northwest main 1 break out-by the face. The fall was approximately 40'L x 18'W x 10'H.
An empty haul truck traveled in a forward direction through a dump berm and came to rest at the bottom of the angle of repose dump fatally injuring the operator.
The employee was present while two contract workers were performing a repair on a centrifugal water pump. During the repair, an explosion occurred. Heat may have been applied during the repair. The employee suffered fatal injuries.
Truck driver entered empty bulk trailer tank for unknown reason, and was later discovered unresponsive inside the trailer.
Employee was shrink wrapping a pallet with a weed burner and for some unknown reason he burned himself and was found by a co-worker in a serious burn condition. He is burned over 36% of his body including left arm, which has been amputated, and his right upper arm, chest and face. The cause of death was mucomuyosis (fungal infection).
Miner suffered a fatal injury after being pinned underneath the raised cover of a Metso Lokotrack crusher.
Two shaft repairmen had finished pulling lacing from the pipe compartment, the all clear/tucked in verbal command was given twice with response. The bell signal was transmitted to raise the skip. One miner's fall protection lanyard was accidentally left connected to a shaft anchor. As the skip raised the lanyard remained secured pulling the miner between the skip and shaft timber.
Injured employee was helping install roof bolts in a coal face during longwall recovery operations. His left index finger became pinched between the head of a roof bolt and a bearing plate causing the finger to be severed at the end near the first knuckle.
contract employee severed right index finger while performing task of emptying used oil filters into his truck. He also broke his right ring finger as a result of this pinch point injury.
While a pump was being hoisted it contacted overhead grating, in attempt to guide the load, the employee pinched his finger between the crane block and cable.
EE was injured while retrieving a part no. from the steam fan sheave wheel removed the guard as the fan was winding down - he inadvertently contacted the belt which drew his right hand into the sheave wheel and cut the tip of his thumb off. EE was treated by a physician who sutured the wound and placed him on restricted duty. Follow-up Appt. scheduled in 10 days.
During the previous 3 days (June 16-18), EE had been working on a project pulling a lot of hose. He felt discomfort and pain from the pulling in his groin and behind the belt area. He though it might just have been indigestion, but the pain increased and was reported on 6/19/14.
Employee was at the bottom of the service shaft helping unload a longwall cell plate off the cage onto a flat car when his finger got caught between the plate and the clevis attached to the car causing him to mash and amputate the tip of the index finger.
Holding grease hoses together to grease take up bearing. Hoses came apart and grease from pressure gun went into right ring finger.
While repairing a miner waterline rib rolled onto employee. Broken left lower leg & spinal cord injury.
Worker stepped down off of ladder, walked over to get a small bolt, as walking back to ladder felt like air in lower back. As worker climbed ladder, 2 steps up, felt pain. Continued to feel pain.
Employee was mixing a bucket of backing material when some splashed on his legs.
Employee was operating a machine that fills polypropylene bags with pulverized limestone when his left hand got caught in one of the straps attached to the bag, as the forklift operator was backing out with the pallet that the bag was sitting on resulting in the employee's left arm being pulled causing a strain to the machine operator's left shoulder.
3rd shift was belting back and the belt ends were spliced. The splice hit on top of top roller. Injured employee removed top structure and placed on right rib and left foot. Sharp edge on belt structure cut hole in boot resulting in a puncture wound to the top of the left foot, just above his big toe. Open fracture to bone on left foot just above big toe.
Employee was changing flights in a vessel when he felt pain in his lower back. He finished his shift and went to the doctor the following day. He was released to return to work following this doctor visit. Afterwards he continued to have pain and sought additional treatment on July 6 where he was given one day off work. He has also began seeking chiropractic treatment.
Employee was exiting portable toilet, tripped, and fell to the ground. The employee was evaluated at the local health facility and found to have a fracture to her right ankle.
The water truck was positioned under the water spout, the trainer was exiting the cab to turn on the switch to fill the truck, and felt a pain in his lower back.
Employee was putting a trolley pole on the trolley wire to move a 5 man ""jeep"" at powhatan #6 mine. When he went to place the pole onto the wire it missed the wire and he lost his balance and brushed up against the energized wire with his shoulder and part of right arm. he was shocked with energized trolley wire. He was wearing insulated gloves at the time.
Employee was cutting the face of # 9 entry with a continuous miner when a piece of carbide came off a bit and hit employee's face just below his eye.
The employee was watering roadways. When she made a left turn from a crosscut to the entry the water car contacted the rib. The employee had her left foot outside of the cab line and her left ankle was fractured.
He is a trained loader operator who was wearing all of his personal protective equipment including metacarpal gloves in good condition. He was hand-pulling miner cable slack. He grabbed a cable splice and received an electric shock.
Employee was loading JennMar Monster Mats on to roof bolter and felt a pain in his groin area. *Employee did not start missing work until 7/8/2014.
He is a trained rib bolter who was wearing all of his personal protective equipment. He was walking past a shuttle car that was turned off. The shuttle car operator energized the shuttle car and when EE stepped onto the cable, the reel picked up the slack and threw him onto the ground.
Mechanic was removing and replacing bit and sub on drill steel when it shifted. Thumb got caught between steel and nylon sling. Mechanic sustained broken and laceration to his thumb.
Victim was performing normal tasks (hosing decks, positioning barges, testing sand samples, etc.) When he began to experience lower back discomfort.
Steam cleaning equipment, coupling blew off steam hose causing hot water to hit chest (left side chest wall)
He is a trained motorman who was wearing all of his personal protective equipment including metatarsal boots of good condition. He was walking along the side of his motor to hook up supply cars. He ducked under a waterline and tripped over a loose cement block landing on his right knee.
Worker was climbing down access ladder, slipped and fell landing on his right leg, breaking his femur.
Employee was throwing miner cable over on the rib when he slipped and fell. Employee stated when he tried to put pressure on the front of his foot, the back of his heel hurt. Employee continued to work until 7/1/2014.
Employee carried (4) 10' sections of auger up a few steps in the screen room mill building to assist maintenance with the replacement of the augers. He felt pain and strain in his left elbow.
Employee slipped and twisted his knee while stacking bags
employee was cutting at incline conveyor with a torch. Slag fell off onto torch hoses causing them to catch on fire. Employee was burned on stomach and palm/thumb of right hand.
The accident/ injury was not the result of compliance with rules or regulations, mining equipment or systems, skills, proficiency, training, attitude, protective items related to clothing or protective devises. The miner was walking and was not concentrating on the task, stepped in a hole and sprained his lower back.
A scoop struck the end of a track rail causing the rail to shift striking two employees on their legs below the knee. One of the employees was air lifted for medical treatment because of a 4"" laceration he received after striking the top of his head on an airlock door. The other employee received fractures to the tibia and fibula of his right leg.
The employee was dismounting a Caterpillar D10T dozer when he tripped and fell injuring his back and neck. The mines went on two weeks vacation from 28 JUN thru 11 JUL. The injured employees first scheduled day back was Monday, 14 JUL 2014.
While removing stone from conveyor belt the employees finger was caught between the two causing the tip of his right middle finger to fracture and require stitches.
putting plastic pipe down air-shaft, pipe swung around & hit under ribs. Bruised ribs & liver.
Kenworth blast truck was being used to load holes on a shot on the G haul road when driver lost control and rolled down a steep ramp tipping truck up onto its side at the bottom of the ramp. The driver was pinned under the steering column for approximately 2 hours. Employee was transferred by helicopter to Eastern Idaho medical center.
Employee was lifting shovel cable & later exp pain & swelling in both hands.

One of the employees was air lifted for medical treatment because of a 4"" laceration he received after striking the top of his head on an airlock door. The other employee received fractures to the tibia and fibula of his right leg.
Pinched the left foot between the relay bar and mine floor when pulling shields on the Long Wall face, trained on pulling shields, wearing proper PPE.
Employee stated that as he was tramming the diesel scoop in the haul road at break 46 he ran over a rock that caused him to strike his head against the canopy inside the operators compartment causing pain in his neck and back.
While carrying supplies to the continuous miner, employee hit his right ankle against the digging arm of the loader. Became LTA on 6/29/14.
Employee was lifting a sample bucket out of truck bed when he experienced pain in his shoulder & arm.
Using a shovel to remove rocks the employee felt a sharp pain in his abdominal area. His supervisor was notified. July 3rd, he stated that a hernia had developed and was diagnosed with having sustained an abdominal hernia. July 14, the surgical report was reviewed and confirmed that the injury sustained was linked to the employees work activities on June 26th.
While setting a hydraulic floor to roof support, as the top contacted the roof it caused the bottom to kick out striking the employees left foot causing a fracture to his 4th toe.
While walking, employee stepped on a rock and twisted his ankle, causing a sprain.
Pry bar contacted right forearm of employee.
Miner was pulling on a ""Hog Chain"" when he felt a pain in his abdomen. the miner was later diagnosed with a hernia and did not start to lose time until 8-6-2 014. the following were not involved: rules and regs., equipment or systems, skills, training, proficiency, attitude, clothing or protective devices.
Employee was carrying a jack around the side of a 636 scoop and stepped on a hose and rolled his left foot, feeling something pull in the underside of his arch. Diagnosed with a sprained left foot. No problems with training, compliance, mining machines or methods, or compliance issues.
Employee was checking his sights when a piece of rock fell from the mine roof and struck him on the head, left shoulder, and left arm. The injury resulted in a contusion. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
While operating a shuttle car the operator struck the rib which resulted in the operator striking his head against the shuttle car canopy. The employee thought that he was suffering from an off site injury he had experienced previously. It was determined later that it was very likely that the stated incident was the cause of his problem.
Employee was loosening a rope ratchet on the 1J longwall when the ratchet moved suddenly causing him to injure his right shoulder
Employee was ducking under the dryer to look at gear when a bolt sticking out of the dryer knocked off his hard hat. When he reacted to catch it, another bolt struck him on the top of his head.
Employee was walking on the 3 west belt when he stepped on a rock and sprained his left knee. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
While shoveling from under a beltline at the #2 mill the employee felt tightness in his lower back.
Inspection/access door with bottom hinges not supported after being unlatched. Damaged portion of hinges not visible with door in closed position. Miner was positioned to the side of the door, hands resting on solid surface. Weight of door broke the hinges after being opened and fell on top of miner's fingers. Result: two broken fingertips of left hand (index and middle fingers).
Employee stepped on D11R track and opened right side engine compartment door in order to repair air filter housing lid, employee stated that he does not know if he stepped back or lost balance when he opened the door, but he fell between the push arm and the track and landed on the tilt cylinder mount bracket on his left side.
Employee was completing his normal daily task of cleaning around conveyor 19 when he heard a noise coming from a return roller. Employee reached up to a return roller to remove a rock. The return roller caught his sleeve and pulled his arm into the roller causing the belt to burn his right arm.
Tripped in the quarry. Dropped drill hammer on right knee.
Employee was operating a scoop with an interlocking double door. The safety pin was not installed correctly into the hasp and the doors opened while the scoop was in motion and struck the employee in the lower back.
Employee was opening the drain valve to drain tank on water truck. He turned to get away from water and ran into the platform for the fire hose stand.
Employee tripped and fell to the ground and fractured left hip. Employee was working in the Oil Hole and tripped on a oil supply hose in the area.
Employee was scaling top and a rock fell on his foot causing bruising and swelling.
Employee was unloading belt structure rails. Rail bounced off structure hitting employee on right leg and right rib causing injury.
Employee was sliding a track tie underneath the track when he felt a pain in his lower back. (LTA effective 6/26)
He was a trained utility man who was scaling roof material by the miner on the 13 East Miner Section. He was wearing all of his personal protective equipment including his metacarpal gloves. As he was scaling, a piece of material fell out of the top and struck his right hand.
EE went to plant foot while throwing a roller through a door and rolled ankle, causing a RT Ankle and Foot Sprain.
Pulling curtain off the head of the miner, felt pain in lower back.
Employee was using a chain hoist and a 3/8 chain to lift a side of a screen to change the springs on the right side the chain around a beam holding the hoist broke and the screen fell forcing a spring out hitting him in the right knee causing him to fall.
Employee was working on dilute pump. While he was positioning wear ring to go in pump the wear ring slipped and caught his right middle finger between ring and shaft. He received 3 stitches on his finger.
Employee stated he was throwing the CM cable out of the roadway when he felt pain in his lower back.
While fueling a loader, staged away from the quarry, fueler slipped on ladder attached to loader, falling to ground, injuring elbow. Weather conditions were wet/rainy and fueler's gloves were wet, causing the fueler to lose his grip on the ladder.
Employee was dragging canvas in E26 section. He lost his balance and fell, hitting the right side of his body / rib area on a ventilation tube. He was diagnosed with one cracked rib. LTA effective 6/25/14.
Employee was bent over to lift a floor plate from drainage ditch. Pulled lower right side of back.
Injured employee stated he was preparing to drill a bolt hole, put the drill steel against the roof and as he was withdrawing his hand a piece of rock fell and struck him on the right hand resulting in a laceration to his index finger.
Employee was building framework for air lock doors by himself. The frame started to fall and he tried to catch it, straining his right shoulder. He continued to work all scheduled shifts up to July 21st. His first scheduled shift missed was July 27. He is part of a weekend crew.
Injured was carrying a sieve with a co-worker. Injured lost his grip on the sieve and it dropped onto his knee causing a laceration. The laceration required 5 sutures to close.
While installing a 16ft roof bolt, employee felt a pain in his left bicep. Diagnosed with a torn bicep muscle.
Walking down an incline walkway along belt stepped on loose rock / coal twisted left knee, Trained on walking about the mine, wearing proper PPE
No event. Employee was crossing the belt at break # 43 on the # 2 belt when he felt a pain in his right knee. Employee sought medical attention on 6/21 and again on 6/24. Each time the employee was released to return to work. On a follow up visit on 7/8 the employee was taken off work due to continued pain in knee.
He was spraying to kill weeds and felt something in his clothing. He knocked out a spider. Later, at home, he discovered he might have been bitten by the spider. He reported it on Monday, 6/23/2014.
The operator was holding the tail line in place while being cut from the dredge. While holding the line, the line was cut, the line jerked pulling the operator's right shoulder out of socket.
Employee decided to use gasoline when burning garbage.
Employee was cutting a limb from a tree when his saw kicked back and cut his hand.
Miner was in vehicle maintenance wash bay cleaning out a truck bed using a 20' pike pole and a water hose. Miner jabbed pike pole into a small rock stuck in groove between welded plates. Rock didn't move jarring left hand and resulting in a sprain to the left wrist.
Closed door on right index finger and cut tip of finger.
Bent down to retrieve hose and then stood up and was preparing to lift the hose to mechanic on top of drill. Blacked out and struck head on rock upon falling to the ground.
Employee was on tail piece crossover when he stepped down off and twisted his left ankle.
Two employees were performing maintenance on a hazmag crusher and while flipping a blow bar, it fell off of the stud on the lifting mechanism towards ee who was standing on the belt facing the rotor; he was able to keep his hands on it as it fell, however at approx 600 lbs it did hit his left leg which pushed him back up against the back wall of the crusher.
Worker was stacking rock onto a pallet at the chopper shop. When he started to place another rock onto the pallet the load of rock shifted. This caused the middle finger of his left hand to be caught in between two moving rocks and this resulted in a fractured finger.
While carrying two, five gallon buckets of garbage from the Motor Barn to the Mainline track, the employee rolled his ankle at the de-rail. Employee sprained his right ankle causing him to fall. The employee continued to work until 06/25/2014, he then missed three scheduled shifts.
Employee was assisting fellow employees in removing belt structure on the 8 right longwall. While lifting a bed rail for the conveyor belt he felt pain in his back.
Employee stated that he was walking beside the section power center when he tripped over the feeder cable and fell to the ground. This resulted in a sprain/strain of the lower back.
Employee was lying on a creeper while spraying belt structure with a water hose. The high pressure stream hit a bottom roller causing the stream to bounce back and hit him in the face, allowing material into his eye.
The employee was changing a 440 volt starter. Turned power back on to check starter after replaced. Checked the on/off button and determined new starter worked. He did not shut power down again, elected to hook up motor leads and in the process came in contact with the relay which in affect pulled starter into position causing flash back injuring him.
EE is a dump truck driver/owner operator working thru T407. As he was on standby work due to a blasting operation, he was staged outside the pit area and in the North/East area of the quarry. While standing outside of his truck on a pile of material, he proceeded to step off the pile and lost his footing, thereby falling onto the ground.
Employee reported to me at 7:27 AM that he stepped on a rock and twisted his right ankle. Refused treatment at that time, because he didn't think he hurt it that bad. On 6/20/2014 at 11:05 AM, he stated that his ankle was still sore and hurt. Advised employee to have x-rays immediately and seek treatment. No witnesses to the accident
The injured employee alleged that he was stepping off the haul truck ladder and his left foot went into a washed out rut. He then stated that he felt a pain in his left knee.
The EE was dismounting the R65 Haul Truck when he lost his footing and he fell to the ground. He landed on his right heel and sprained his foot. He has been placed on restricted duty. New Diagnosis of broken heel instead of sprain
While helping the bolter operator, he got his left foot caught by the boom of the bolter, causing dislocation of his left big toe.
EE assisting manual extraction of plug at 1st stage splitter box. EE felt lower back pain. EE self-treated until 6/23 and sought medical attention on 7-day break. On 6/27 EE missed scheduled work shift. Back strain complicated by compression of vertebrae.
Employee was attempting to remove pin on clevis (on raise bore head) with a hammer, when he struck himself with the hammer on the left thumb. X-rays revealed fracture to thumb.
Employee was walking in quarry when he slipped on a rock causing a cut on the front of his left leg.
He was a trained mechanic who was splicing a cable on the 12 East Miner Section. He was wearing all of his personal protective equipment including boots of good condition. He slipped in the mud and twisted his left knee.
Employee was scooping the roadway when his canopy hit the ventilation tubing causing it to come into the cab hitting left elbow.
While passing trough an airlock, the employee claims to have twisted his left knee. This incident occurred due to the lack of concentration on what the employee was doing. The incident did not involve any mining equipment, nor any aspect of compliance with rules or regulations. The employee's attitude and lack of concentration contributed to this incident.
As the EE was helping to change screens the lifting hook came off of the anchorage point and the screen struck the EE on the head. The EE was wearing a hard hat minimizing the injury. This incident occurred on Wednesday June 18, but the EE did not seek medical attention until Friday June, 20 and was placed on modified duty at that time, hence making this a reportable incident.
Employee was up on the landing with all other employees pulling tarps over rock pile. A rock came out from under him and he fell and sprained wrist. He went to the doctor the next day.
Employee was loading block into a scoop car bucket and got the little finger of his left hand caught between the block he was handling and a block that was setting in the bucket. The employee fractured the little finger on his left hand as a result of the incident.
Employee received a concussion when he fell onto a low boy dolly out of a manhole.
The employee was assisting with changing screens on a portable screen plant. Two employees were handling one screen panel weighing approximately 50 pounds when one employee felt pain in lower abdomen. Employee was sent to physician for a check and was diagnosed with an Inguinal Hernia. Consultation with a general surgeon was scheduled and he returned to work on restricted duty.
Employee was organizing top shelf in the tool room and while stepping down from the step stool, the stool slipped out from under him.
After torching holes for guard rail in 20 block spur (23D), injured stood up and started to walk away, stepping on a piece of lump coal and rolling his right ankle. Finished shift, diagnosed with ankle sprain the next day. No problem with training, PPE, mining methods or equipment, or regulatory issues.
EE was placing paperwork in truck that was loaded on lowboy trailer. When getting out of truck, he mis-stepped & twisted his knee. EE was able to continue to work for remainder of shift, but could not return the next morning due to swelling & pain. On 6/18/14 Dr drained fluid from knee, told EE to rest for the remainder of week & to wear knee brace until knee was completely healed.
When re-railing a belt winder individual came in contact with trolley wire and fell to the ground striking head on winder. newly trained on job task, wearing proper PPE
Ee was walking and stepped over a parking ditch and heard a pop in his left calf. He had pain. He was evaluated at the clinic and placed on restricted duty and the leg was wrapped. No restricted duty available, so the employee lost 3 days. Released to full duty 06/25/14.
Employee got off the longstick to grease the bucket. When he was getting back on, his foot slipped causing a muscle to pull in his back.
Employee stated that he and another coworker were carrying a timber when he felt a pain in his lower back causing him to fall to his knees. Employee Retired effective 6/23/2014.
Height condition changed from 6.5 feet to 5.5 feet. Employee struck head on top, causing pain in neck and lower back. Employee continued to work until 7/18/2014, when doctor took him off work.
Employee was attempting to reverse a set of equipment doors. The doors were removed from the frame. The plaster and wood was removed from around the frame. the employee attempted to push the frame over when the side support stuck him in the chest. the employee suffered a contusion to his chest.
Employee was changing a 15"" tire on a skid steer when the tire fell on left knee.
While bolting top the drill steel broke swinging back and striking Employee in the right hand thumb area. Employee did not seek medical attention until 6-19-14 and was diagnosed with a fracture.
EE advised us letter dated 8/4/14, was received by employer on 8/8/14, illness to lower back is related to wear & tear from 13 year tenure at Greystone Quarryes, Inc. Attached copy of letter for reference. E-mail, ee advised us, as per his doctors communication and the x-ray results, the correct date for the injury/illness is 6/17/14. See attached copy of the email for reference
He was putting up a roof bolt. The bolt bent. He accidentally hit the rotation. The bent bolt hit him on his left wrist resulting in a fracture.
Digging rock debris from underneath the hopper of a conveyor belt. Pushed shovel into pile of rock and felt a small pop and then a sharp pain went through his right arm.
Employee stated that he was operating #37 jeep coming out of the rock dust hole on the mine bottom when the trolley pole came off the wire. As a fellow employee placed the trolley pole back on the wire coal dust and rock dust from the mine roof entered the employee's left eye.
Employee was riding mantrip when he was caught between the mine beams and the mantrip. He pulled a muscle in his lower back.
Employee was standing beside the flat bed truck when another employee was attempting to unload a ram car boom jack. The jack spun around on truck and fell off truck and one end of the jack struck the employees left foot. Fractured foot.
While walking at sand plant, employee slipped on algae that had grown on concrete floor and fell. The fall resulted in a sore wrist, later diagnosed as a sprained right wrist and sprained hand.
Employee was driving a water truck and stepped out and twisted his right knee on un-level ground.
Hanging face curtain in #2 entry on 4ft ladder. Ladder leg buckled and employee fell from the 3rd rung on the ladder landing on elbow forcing shoulder upward.

While leaving the bathhouse ee told me he hurt his knee, I asked him if it was ok, and if he wanted to fill out a accident report and he said no that he was ok , they wasn't never a clear event or incident to how this employee was injured if he was injured at this mine. Note this was his second day at the mine, his first day was finishing his training.
Employees working on screener in stone plant. After de-activating the power supply and locking out, went to remove the ""v-belt"" from the drive motor and had to use a wrench to brace the belt back from the motor. Tension from the pulley released when the employee completed a full rotation causing the wheel and belt to turn freely catching his finger between the tool and machinery.
EE on the ground changing batteries. Co-worker started the 785C Service Truck and got out of the cab to tell him to turn off the welder. The 785 rolled back and trapped his right leg and foot between the bumpers of the trucks. Co-worker got back into the cab and put the 785 in reverse to get the bumper off of his right leg and foot. He took injured EE to the hospital immediately.
Two employee's were was setting a liner on a cone head. A telehandler was resting the liner on the head. As it did, the liner slid off of the cone and rolled on the ground pinching an employees leg between it and concrete blocks that the crusher sits on resulting in a broken left leg.
Employee was walking down mill #1 ramp, looking down at his 2-way radio, when he stepped onto a big rock, slipped and caught himself, he felt his back ""pop"".
While stepping up onto Jumbo drill to start drilling, employee rolled his right ankle in the process. Xrays were positive for fracture. Restrictions were issued.
Employee was bolting when a small rock fell through screen & contacted his finger on right hand.
While center bolting he was holding the bolt and reaching for a drill steel and felt a pop in right shoulder. On 7/1/14 his restrictions changed and we were not able to work with them.
On 8/8/14 the employee was unable to return to work at full capacity due to an injury that occurred on 6/1/14. On 6/14/14 the employee attempted to insert a roof bolt into the drill hole and shove the bolt through the resin cartridge by hand. The action strained his right shoulder.
Employee was getting into a man trip when he felt a pain in lower back.
Employee was lifting boxes out of totes. The employee states after lifting a box he felt pain in his groin. After medical treatment the pain was revealed to be coming from his hip.
Roof bolter operator was swinging drill head to install a roof bolt when he caught his finger between the boom arm and drill pot of roof bolter.
Employee moved a belt frame that was leaning against the coal rib when a piece of coal approx 2' x 3' rolled off rib striking the side of employee's knee causing MCL tear in his knee.
Employee stated that he was operating the continuous miner in the face of the #5 entry. He had taken the 1st sump in the cut sequence and had positioned the off standard continuous miner for the second lift when a section of draw rock fell from the top between the roof bolts and the rib, striking him in the back of the neck and left shoulder blade area.
Employee was dismounting a D-10 Dozer. He stated that his foot slipped off of the bottom step and he fell to the ground. The ground in this area was level, dismounting hand rail and steps were in good condition. Employee was wearing his gloves and hard hat at the time of the fall.
Employee was assisting with loading a fan onto a flatbed and load shifted, pinching the tip of his right ring finger.
Employee was working on roof bolter leveling arm chain, chain broke striking employee on bridge of nose and right eye causing injury. Employee was transported by ambulance to hospital.
EE was drilling holes closest to the edge of the 3775 bench. The bench had a contour, EE misjudged his clearance and drove the drill off of the bench. The drill went over sideways to the next level 3760 bench. EE was ejected out of the window and fell to the ground approximately 12 feet.
Individual was installing ventilation tubing to the face of the 9 right section. While bending to lift the tubing he felt sharp pain in his right groin area. A hernia resulted from this incident. The individual has had surgery to repair the hernia. The operator was not notified of the incident until 7/3/14
Injured was building a crib on top of an overcast at 34 block 1 North. When he went to pick a crib block off the ground to install it another crib block was thrown up on top of the overcast pinching his right ring finger between the cribs resulting in a commutated compound fracture of the distal phalange. No issue with PPE, mining methods, machinery, training or compliance.
Employee called supervisor in AM on 6/13/2014 (which was a scheduled shift for him) and said that his left knee is sore & he was going to doctor.
6/12/14 EE reported back strain from oper. FEL. EE did not see Phys. until 7/1/14-ordered off of work for 2 wk. Follow up visit on 7/15/2014 EE was cleared for light duty starting 7/22/2014. Phys. has not determined if work related or existing non-work related injury from summer/2013 and again 3/2014 -incident being treated as reportable pending diagnosis.
While conducting maintenance activities the employee slipped on a wet surface and fell resulting in a fractured ankle.
Employee slipped walking up the steps on Plant # 2, heard a loud pop in right leg. Steps were clear and weather was not wet.
Employee was winding the rope back on the spool after pulling on conveyor belt on a fresh belt extension. While he was in the process the scoop moved forward and his left foot was caught under the scoop bucket. This resulted in a fracture in his left foot.
Employee was walking at the plant close to the 3"" minus rock pile when he stepped on a rock and twisted his left knee.
Employee was installing the top rib bolt in the #3 Heading on the 004 MMU and felt something pop in his left shoulder. He started missing work on 6/23/2014 pending the results of an MRI.
Employee was putting away a full size compressed gas cylinder when it fell and struck his left foot.
On 6/12/14 employee called in & said his back was hurting & couldn't work. He thought it was a pre existing injury of his sciatic nerve from 2008, he called back on 6/23/14 & said he wanted an accident report filled out due to jarring in a rock truck. He was off several days before reporting that he was injured at work.
Bending over to pick up stone while stacking pallets and back began to hurt
Employee had just cleaned the coal feeder with a 488 scoop and he backed into the intersection at the feeder and got off the scoop and stood by the scoop bucket when the shuttle approached the feeder to unload the shuttle car caught the edge of the scoop bucket pushing it onto the scoop operators foot causing a fracture to his left foot.
EE was walking and stepped out of scale house turning her left ankle. Was taken to Hazard ARH and diagnosed with a severe sprained ankle. Returned to work.EE worked til July 1st with no restrictions doctor excused her from work because of swelling in foot.
Employee started to drill hole when a rock broke and fell striking employee's right hand (middle and ring finger) cutting them.
Employee was removing a belly pan from a d-11 dozer. a bolt used to lift with broke causing the belly pan to wedge employee's right ankle between the belly pan & the framework of the ripper
Employee stated that he and another employee were carrying a 34"" x 34"" piece of sheet metal up the belt catwalk. The employees foot slipped causing him to fall against the hand rail resulting in a back sprain.
Employee stepped out of mine office and felt pain in his back. Employee worked the rest of the day. He went to the doctor on 6-12-14. He went back to the doctor on 6-20-14 and was taken off work until further notice making his first day of loss time as of Monday 6-23-14. *Employee returned to work on 7/14/14 and went back off work on 7/21/14 as a recurrence of old injury.
While greasing bucket, climbed on top of bucket, slipped & fell into bucket (about 4')

Employee was operating the continuous miner when the coal rib rolled out onto employee's foot and ankle causing hair line fracture to ankle.
Struck with drill steel in the right side of head and arm, trained on roof bolting, wearing proper PPE
The miner was doing routine maintenance on the screen plant which required him to be on a ladder to check for excessive heat. While doing this the ladder shifted and the miner fell breaking his left wrist. The miner was three and a half feet up on the ladder when the fall happened. He was transported to a hospital and received medical attention for his injury.
EE was tasked to open a hinged access door. After the 4 pinch style clamps securing the door were removed, the EE started to open the door at which time the hinges broke and the door fell approx 3' pulling on right arm and shoulder. The EE was evaluated by a Physician and surgery was performed on 10-29-14.
Truck driver was leaving dump. It was raining and bad wind storm blew two trees down, one on truck one on trailer. The one on truck most of impact hit on passenger side. Driver shook up thought he hit head. At that time took driver to Lonesome Pine Emergency room. He was checked out and released - gave him one day off because of soreness.
Employee was driving a shuttle car down # 3 entry on # 1 unit and ran over a unknown object, he felt pain in his lower back.
Taking a pinner steel out of the drill hole from the mine roof, and strained middle lower back on left side.
Employee was welding and raised his hood when another employee began grinding in the area. Employee eye became irritated over the course of the week, but kept on working till 6/16. Went to eye doctor on 6/16 and there was a small piece metal in his eye. Safety glasses were being worn under the hood.
Employee was assigned to install the grilled plates on the IKN of the cooler; in the process of installing the plates he felt a sharp pain on his lower back when he extended his body forward during the process. The employee was wearing his PPE.
Performing maintenance on the 1A conveyor. Propped the step ladder against the tower holding up the tail of the conveyor. Ladder was on uneven ground. Climbed to the 3rd step (36"") placed left foot on the tower, step ladder kicked out and she fell to ground.
Employee was installing a conveyor belt clamp, and was holding the clamp when it got hang up. The clamp was being moved by a forklift, fork got hung up released and jarred him. This release jarred him and hurt his shoulder.
Employee stated that he was cleaning up the pad and side walk outside the mine office. He stated that he was picking up small roll of belt and felt a sharp pain in his lower back. He was transported to the hospital and was treated for a pulled muscle and became a lost time accident.
Pulling on belt rubber with channel locks strained shoulder.
Employee was operating a dozer on solid rock when he got jerked causing a hurting in his lower back.
Employee reported he stained his back while replacing anvils in CEMCO Model 80 crusher.
Employee was in the process of installing a beam on the mine roof. The beam was fastened by chain on one side and was being lifted up on one end to fasten it to the top when the beam rolled inadvertently catching his right pinky finger which resulted in a fracture and laceration resulting in stitches.
Employee experienced back spasms throughout the day. Was diagnosed with muscle spasms and taken off work.
After racking out the 4160V switch gear, employee used a hand-held meter to ensure no power to blown fuse when an arc flash occurred.
Injured employee & another employee were removing gearbox on conveyor. Injured employee had gearbox in hand, turned to set it down when he felt pain in lower back. The other employee said he heard a pop.
He was performing general labor, which he is trained in. He used proper lifting techniques. He was wearing all of his personal protective equipment. While carrying a jack he felt pain in his lower back.
Employee was using come-a longs to pull a top rail when the chain slipped on come-a longs causing rail to fall on Employee's left knee. Employee was treated at Raleigh General Hospital.
Employee was in the process of replacing the drag ropes on Dragline, he was removing the u-bolts that attach the new cable to a wooden spool. As he pried the u-bolt out, the cable spun on the spool and the end of the cable hit him on left shoulder resulting in a fractured shoulder blade. As he fell he sprained his left wrist also.
Employee a continuous miner operator on the #1 Section was injured when a roof rock fell and struck him. Employee was standing in #6 Entry approximately two breaks inby spad 9050 when the accident occurred. The roof support was properly installed and intact. Employee sustained injuries to his pelvic area as a result of the accident.
Employee felt a pain in his right arm while throwing a track switch on Oak Springs bottom. He was later diagnosed with a torn bicep. (LTA 6/16/14)
Employee was on his knees next to the miner at the control panel. Mechanic was working inside the contact panel checking voltage from the master panel to the contact panel. The master control panel and contact panel was swung out. When employee began to rise from his knees he inadvertently laid his right hand on the swing out portion of the contact panel which was energized.
During roof bolting operations the wrench became stuck on the head of a roof bolt. While attempting to remove the wrench the miner's left index finger was caught between the drill mast and the wrench resulting in a laceration.
The employee was walking up a covered staircase, that leads to the hourly parking lot. The employee's foot slipped off the front of a step which caused him to stumble onto that foot awkwardly. The incident resulted in a fracture to a metatarsal bone in the employee's foot. No defects were found during an inspection of the stairwell.
Employee fell approx 24"" off ladder resulting in a laceration to the back of the head.
The employee was taking the lid off the rear of a scoop to wash it. Another scoop operator was turning into the area to wash his scoop. The employee was pinched between the two scoops.
The anchor bolt pulled from the wall causing slack in the come-along chain which free wheeled the distribution box that came down on to Employee's right foot causing a fracture.
Hit his hand off side of MBC while getting out at the end of the shift. Sought treatment of injury on 6/30/14.
Employee was observing the continuous miner and a piece of draw rock fell and struck the back of his leg and ankle. Fractured ankle.
Employee was attempting to reach a mine cable that needed hung higher on the mine roof, and attempted to climb up on the trailer of a Diesel lubrication machine (Lube Truck), at which time he slipped and injured his knee.
The miner stood up after squatting to look at a pipe and felt pain in right knee.
Employee stated that at the end of the shift he staged the shuttle car up at the feeder and dismounted to walk to the mantrip. As he bent forward to walk, he felt pain in his back. This resulted in sprain.
The employee was operating a shuttle car, hit a bump and bounced in his seat and then felt a pain in his side. The result of the injury was a strain to his lefty side ribs. The employee did not start to lose time until 06-17-2014
Injured worker states that he stepped off loader and twisted knee.
Employee got off the ride and was walking toward the power center when stepped in a hole and his right knee gave out. Employee tried to catch himself on the power center hurting his right shoulder.
Front End Loader operator was descending ladder to take a break when he rolled left ankle as he stepped onto bottom rung. Ladder and operator's boots were both clean and free of debris. Operator stated he maintained three points of contact as he descended ladder. Operator could not recall looking at rungs as he descended. There were no witnesses to accident.
While hand scaling in the face the scaling bar slipped on a rock and caused pain in the employee's right arm.
Employee was changing tire on a battery scoop. Scoop dropped four inches pinching employees hand between top of tire and the fender on the scoop.
The injured employee was riding in a diesel man trip conveyance traveling to section when the man trip slid off of a bridge causing the vehicle to rise up abruptly fall back striking the mine floor. The resulting force caused him to feel immediate pain in his lower back. He was diagnosed with ruptured spinal disc. The injured had a previous history of back problems.
Loosening bolts on #2 Elevator and tightening chain. Pulled right side groin
IE was bending a roof bolt when he twisted his right knee. IE has worked full duty up until surgery on 07/31/14.
Employee stated that the motor crew was picking up garbage along 9right track. At 5 block employee was pulling on a piece of curtain about 10 ft. long. He felt pain in his lower back.
Employee had shut a manual ball valve off on a liquid lime tank and started to uncouple the line at fitting due to line being blocked. When hose came loose the line blew liquid lime in his eyes and mouth due to the handle turning on shaft of ball valve and not closing.
EE was washing and cleaning up a primary impactor to paint on 6/5/14. Removed a wedge from side access door, set it on frame to do more cleaning, wedge dislodged falling onto main frame then glancing off onto his foot and ankle. He had pain but continued to work. 6th he came into work with foot swollen and having a hard time walking. Went dr 6/9/14, major contusion, off 3 weeks.
EE was removing metal band-straps from a pallet of 50-Ton Heintzman Jacks. As he cut the last metal band-strap, one of the jacks sprung out and landed on his right foot. EE sustained deep contusion.
Another employee was beating a bearing with a hammer. EE was standing next to the employee watching when a small sliver of metal flew out and hit him in the hand. He was not wearing gloves and there was only a small abrasion. He removed the metal and stated he did not need any additional medical attention.
While performing a pre-PM on a haul truck, employee was walking towards the back of the truck, when he stepped on a fitting on the floor, which caused him to fall and twist his ankle. He was taken to the ER, then his PCP, referred to the orthopedic specialist, and diagnosed with a bad sprain, and taken off work. Currently ONGOING.
Employee was working at the rock splitter cleaning between rollers and a rock came down the roller table crushing pinky finger on right hand.
Holding grease gun bent over to grease 988B loader and felt back pain.
Employee was putting miner chain back together; put connector in, co-worker was knocking pin into chain. Co-worker hit the pin causing something to break loose hitting employee in right eye under safety glasses. A piece of metal was discovered by a doctor on 7/30/14 and procedure to remove it was done on 7/31/14. Employee continued to work up until 7/16/2015.
While en-route to work, employee's vehicle was rear-ended as it travelled down the access road. Employee's vehicle rolled over before landing in the ditch.
Employee is a stone cutter who was loading a large piece of stone at the stone saw. His hand slipped and the thumb on the other hand was smashed under the rock.
Miner was stepping down onto the last rung of the ladder on the CAT 988B Loader (equip#2746), when the weld of the rung broke. Miner fell approximately 2 feet to the ground on his back, followed by his head. Miner felt ok to work and 2 hours later he felt dizzy. Miner taken to the medical clinic to get checked.
Employee was welding sleds for top belt rollers with another Employee, when he received flash burn in his right eye. Employee was treated at Raleigh General Hospital and also followed up with Dr.

Resealing a 240 volt battery when an arc from the battery strap grounded to the frame burning both hands, wearing PPE, individual was trained on job task.
Truck driver claimed personal injury then came later and stated maybe is from driving with spinner knob. Had MRI completed and found labrum tear in shoulder.
Employee was using a come-along to move over a screw auger trough with a wire rope sling wrapped around it. Whilst pulling the lever the wire sling broke and the employee fell backwards and hit his upper back on the edge of a steel beam. This contact caused a contusion on his back. The doctor's note stated ""ee was seen today in the ER. He should not work tomorrow"".
The employee was removing a high pressure staple with a pry bar. When he pushed the bar it slipped and his hand hit the pan line on the longwall face. His index finger on his left hand was fractured.
He was performing his regular job as a trained shuttle car operator. He was wearing all of his personal protective equipment. While operating the machine, he turned his head left and felt pain in his right shoulder and neck.
An employee who was under a doctors care for his left knee, aggravated the knee while climbing onto a bull dozer. The employee was under no doctors restrictions at the time.
Employee was rock dusting when a small piece of scale rock fell hitting employee in right knee. The employee fell back and injured his lower back.
Emp was operating Volvo end loader when he noticed a flash as he dug into pile of rock. He continued to operate when he noticed black smoke. He then realized a hydraulic line broke and was smoking. He retrieved the fire extinguisher mounted on handrail of loader and bumped his Rt. elbow on handrail. He was sent to Occ Health on 6/4/14 he received restrictions of no work Rt. arm.
Employee was preparing to setup to transfer cement. When preparing to loosen bolts on the cement line employee had a very sharp pain in left shoulder and bicep. Was seen by Dr on 6/19 & 6/26. Dr reported he had slight tear in left bicep/shoulder. Could continue to work without any restrictions. Employee reported he was still in pain. Returned to Dr and surgery is 11/12.
Employee was installing a bucket rigging pin in the upper hoist chain on the company #103 dragline when he felt a pop in his left elbow.
Installing discharge chute on conveyor screen when it slipped pinching the right thumb while aligning the bolt holes.
Employee was checking and changing pins in a pin mill that was locked out. He rotated the drum by hand and caught his right between a pin and the pin mill top side-wall edge. X-rays at the hospital revealed his hand was broken in a couple of places. He is scheduled to see a surgeon soon for plan of action.
Employee was plugging up a 72 volt permissible ride, the employee was turning and twisting the plug trying to make it charge and received a electrical shock.
Bates Contractor was recovering water line and loading it on water line sled. While loading the water line he smashed his middle finger on his right hand between sled and water line.
Miner was loading resin onto roof bolting machine when he felt a pop in his right shoulder.
Employee was changing oil on dredge and strained his elbow.
During the drilling process half of a 2 foot wrench had slid off the side of the tray at the drill station. After the hole was drilled he began to drop the boom causing the wrench to wedge out between the boom and the glue tray. The wrench began to bend and broke free striking the employee in the face; causing a laceration to his lower lip and a fracture to his upper mandible.
Employee was setting wedges under pipe rack. Employee hit below left knee while using hammer, causing bruising
Employee working at Cemex Balcones in Lyston Building moving rocks from Quarry to plant using skid steer. Twisted right ankle stepping down from skid steer. Taken to Texas Med Clinic, diagnosed with sprained ankle, given boot to wear with crutches as needed and placed on restricted duty. Cemex Balcones will not allow employee back to work until able to wear steel toe boots.
EE was moving a vibrator from the pocket of one railcar to the the next for transloading material from railcars to trucks when he strained his back. EE did not seek medical treatment until 6/30/14 where he was treated with Rx meds, time off, and modified work restrictions.
The employee was lifting a 100# bag and lost his grip on the bag with his right hand and tried to correct it with his left hand. The weight of the bag on his left hand caused him to fracture his pinky finger bone on his left hand.
After filling the surface dust tank, he was struck in the left abdomen when a 2"" air hose came loose at a fitting. Diagnosed with a bleed from the spleen, corrected by surgery.
A roof bolter operator, in the process of bolting the #3 entry on the #2 section when a rock fell from the convergence area between the rib and roof. The rock struck the miner in the left waist area causing a dislocation of the left hip.
employee was carrying crib blocks along the track heading and tripped on a track tie. As he fell, one of the crib blocks caught his right middle finger against the top of the rail. (fracture and laceration with stitches)
EE was parked parallel to a Genie S85 man lift. Attempted to pull out from machine when he realized left rear wheels were choked. Exited the truck to remove wheel chocks, truck began to roll backwards. EE was struck/trapped between basket of Genie lift and the left front of truck. EE exited truck with engine running and transmission on neutral with parking brake partially engaged.
Repairing a water swivel when his hand slipped and water swivel landed on his hand

Employee was lifting a 20' length of 2"" pipe. One end of the pipe was stuck in the mud. Employee felt a pop in groin area followed by a burning sensation. He was diagnosed with a hernia on July 1,2014.
Rain soaked, rutted out haul road lead to vehicle hitting hole and causing the operator to bounce in his seat. By his own admission, he was not wearing his seatbelt. Operator was bounced up and came down hard on the seat. Caused lower back pain.
Employee was connecting conduit in a hole he dug and sand fell in on side and hit him in the hip area and injured his hip. He said the hole was only 3.5' to 4' deep but he was on his knees when it happened.
Parts were being off loaded from the service truck by both employee's. Employee stepped on a rock and fell. Part fell from hands of employee hitting the leg which resulted in the broken bone.
Employee was stepping up on the 777 truck #4012 when he slipped on step. Employee claims to have strained lower back and right leg. He reported both are sore.
Employee was cutting some metal outside at the shop when he tripped and fell on his left elbow. He continued to work until he was off work with surgery on his elbow 08/08/2014.
Making repairs to screen table. Was using loader to support table due to not having enough crane to support weight. Hydraulics bled down and table slipped off bucket.
Employee stated that while operating a haul truck, he ran over a rock & it bounced him in the seat. Later in shift, he stated he felt discomfort in his neck. We took him to ER-Full Release was issued. On 6/26/2014, he went to another doctor, who issued restrictions. 1st shift missed was 6/27/2014.
EE was driving a haul truck and complained of back pain. This took place on Sat.6-21-2014. I contacted ee on Monday reference the incident and told him to go to RediMed and get checked out. ee contacted me Monday at app. 1500 Hour and stated he will be off work until next office visit 6-26-2014
Employee alleges back pain from removing spider nuts (approx. 12 lbs.) from the primary crusher. He was evaluated by a physician and taken off of work. He returned to work on 6/24/14 with restrictions

Employee was building a brattice with another worker. He was putting block up to cap off. The employee handed him a block that was too big and when he went to hand the block back down to be cut he lost his footing.
On 6/27/14 the EE indicated that the injury occurred on 6/20/2014. He explained that they had been lying on their backs tightening an adjustment nut for the belt with a wrench. It was an awkward position as they had to pull on the wrench at the same time to get the nit to tighten properly. He felt a little pain in his groin but he said it did not hurt that bad.
After picking up tag line, employee felt pain in right knee
EE was lifting a return roller (40 lb). The ee picked up the roller by squatting down holding it near his body & bending at the knees. When he stood up, he felt a pain in left shoulder/bicep area. He was evaluated by a Physician on 6-19-14 and given a return to work with RESTRICTIONS.
The injured employee and another employee were carrying a piece of wooden blocking when the injured employee tripped, causing him to fall. His fingers were pinched between the block and the floor.
Employee fell to ground from same level (both feet on ground) while cleaning, used hands to brace fall and broke left wrist.
Employee was operating a backhoe when he felt some discomfort in his groin area, around 11:30. The employee had lunch and went back to work. At 2:00 the employee requested medical care. Cause of injury is questionable.
EE was cleaning the grizzly feeder from obstruction with a bar when foot slipped between the grates in the screen and he twisted his knee. Knee injured, put on restricted duty per medical examiner's instructions.
Employee was using a sledge hammer and wrench to tighten bolts. The sledge hammer bounced off the wrench and struck the employee in the left lower leg. He was evaluated by a physician and released with restrictions. The employee did not work his 6/18 shift. He returned to work with restrictions on 6/19. He was released to full duty work on 6/20.
Employee started fire on ground to burn garbage, returned to fire with second bunch of papers. He removed can of OFF from second bunch and saw can of blue paint already in fire. He turned away just as can of blue paint exploded and hot paint was sent into the air, burning employee's arms, right side of face, right ear, lower part of left leg and little spot on chest and back.
Switching tailings dump location. Maneuvering pipe sections to connect, lowering one with a jack, the jack slipped pinching the right thumb between two pieces of pipe.
Employee was in a hurry to get his task accomplished and misplaced his foot and fell a couple of feet to the left connecting forklift.
Injured was removing a bad magnet by hand from rev 1 lower deck with help from a co worker. They lost control of the magnet. Injured attempted to try and regain control causing what was believed to be a strained shoulder. Injured didn't feel medical attention was necessary at the time. Note: 4-8-15 notification was given that the injured would undergo surgery for a torn labrum.
The injured employee was working as part of the maintenance crew assembling and installing new sections of piping. A section of the HDPE pipe hung on the forks of the forklift and the injured employee reached in with his left hand and attempted to free the pipe. The flange released striking/pinching the employees left hand against the forks, fracturing his middle and ring fingers.
Employee was on the screen tower using a hose to clear a plug up of hard pack on conveyor when his foot slipped on wet surface causing him to lose his balance, but not fall. His left knee was twisted as a result. He was treated for a ligament injury and was given a brace, crutches and work restrictions.
Unknown cause of injury. Employee is a mechanic who has had observations of shoulder but has had no medical treatment and has had no restrictions. However, the doctors want to do some repair of the shoulder June 16, 2014.
while working on replacing an arm of the auto package system employee had to kneel on a chain conveyor. when employee moved off the conveyor for break, he felt stiffness in his back. employee did not receive medical attention until 7/10/14 as he thought pain would go away.

Employee was jumping on a pry bar to break loose a coupling controlling a unloading gate on a rail car. When the coupling came loose, the bar fell and he landed on his left leg jarring his knee.

Employee stated while changing a drill bit in a hand held drill, while tightening the drill chuck the employee stated he felt something snap in his upper right arm. an accident report was filed and the employee continue to work. The Employee went to Med Express on 6-27-14 and put on restricted duty. employee Notified his employer of seeking medical attention on 7-1-14
Chronic back condition aggravated by stationary sitting in a one hour meeting (6/5/2014), resulting in muscle spasms. Diagnosis of minor aggravation of underlying condition 6/10/2014.
A maintenance operator was changing a gearbox on a piece of equipment. A associated part was being pry bared out of the way when the pry bar slipped and the part smashed his finger.
The employee pulled down a curtain panel and aggravated an pre-existing personal injury. He seen the Dr. a few days later and was put on restricted duty. It has since been deemed a work related injury.
The employee stated that he was moving a portable fan with the scaler, and he knocked the jack off in the process. The employee shared that he was in the process of putting the jack back on when the tongue of the fan fell on his left foot. Medical evaluation revealed that the employee has 3 small fractures, and his left foot has been immobilized in a cast for healing.
Employee was operating the bag applicator at the pack house. Empty bags were getting stuck in the feeder. The worker estimated that empty bags got stuck in the feeder about 18 times in a short period of time. The worker had stacked approximate 30 empty bags on the table. When he picked up the stack of bags, he felt a sharp pain in his left groin. First day off work was 6/18/14.
Employee was climbing down a ladder after making repairs to a 992D Loader, when the ladder slipped out from underneath him causing him to fall. while falling he made contact with the tire and injured his back/neck.
Miner was cleaning a roller shim for roller placement. He unhooked his lanyard to move to a new location and did not stay 100% tied off. He tripped and fell backwards from the platform to the ground 14'9"" below. The handrail had been removed to allow the roller installation. He broke both wrists and his left rib.
Employee was carrying 40 pound bags of Floc, when he was putting bag into metal container he felt a pop in his left shoulder. Employee did not feel any discomfort until waking up the next day. Light duty work load until 7/22/2014 will be out of work for about four months due to operation.
The connector was hanging steel girders approx. 40' above the ground. The crane operator was swinging in a load girders. The worker had his back turned to the load, and due to poor communication between the worker, the other connector, and the crane operator, the load swung in and struck the back of his knee, which caused the front of it to hit the column in front of him.
Mechanic changing cutting edges on 938K loader. Two maintenance employees stopped and started to help. While installing last bolt on left side of cutting edge, employee put palm on bolt to hold in place while mechanic tightened bolt. Bolt came loose and spun causing cut through glove and employee's hand.
While retrieving a sample of ore flowing off of the crusher belt. The employee's feet sunk into soft material causing her to trip and fall.
Employee was jumping over a conduit ditch in order to help shovel concrete. He slipped and his right foot went into the ditch between the conduits.
While removing a 70 Lbs chain fall from I beam located 10feet on roof, he went to lift the unit and sustained a hernia on left side of lower abdominal area
LHD bucket tilt cylinder hose failed at fitting. Hydraulic oil came in contact with miner resulting in 2nd and 3rd degree burns to left hand, arm and neck. Minor burns to right forearm.
Employee altercation - Altercation is being investigated. No witnesses to initiation of altercation. Independent investigator has been hired by Insurance Carrier to review and assist. To my current knowledge Boone County Sheriffs Department is investigating as well.
Employee was picking up a bit basket, twisting her wrist. On 9/29/2014 employee was seen by Dr. for employee's complaint of pain in shoulder and was dx with a shoulder sprain and was put on restricted duty.
EE was unloading bag dust from one scoop to another and his right knee twisted causing pain. Later in the shift he exited the scoop. While walking around the batteries, his foot got stuck between the side of the batteries and a rock that caused his knee to turn and pop. He sought medical attention and was diagnosed with a sprain/strain.

Employee was putting a rub rail back on the miner in #2 entry on #3 unit when he felt his right forearm pop. He continued to work till seen by an orthopaedic doctor and was put on restrictions.
Mechanic was pulling on a winch cable, with his life jacket on. As he strained to pull the cable, the life jacket strap became overly tight and broke a rib on the right side of his chest.
EE misstep at the top of the ladder access on a Cat haul truck as he was about to descend. He grabbed the handrail and caught himself to keep from falling and strained his shoulder in doing so. EE saw a physician on 6-9-14 and was placed on light duty/use of right arm and prescribed anti-inflammatory & pain medications. Follow appt. up scheduled 6-20-14.
Employee was exiting down the ladder of the water truck, lost her footing and her right hand grip and felt pain in her left shoulder and lower back. Notified on 7/28/2014 of doctor's orders for physical therapy.
Previously reported as first aid-""As employee was throwing 2-1/2"" x 15' sealing water hose off the truck, felt a pain in his right shoulder."" Injury became recordable when employee had surgery.
An electrical general foreman sustained flash burns after exposure to an arc flash. The employee was attempting to test continuity of 480-volt feeders going to a crane disconnect.
Escorting CSA contractors at Sugar Run, picked up a bag of sand by the bridge switch near Sugar Bottom and felt pain in her lower back, radiating down legs. Started missing 6/6/14 after diagnosis of strain/sprain and prescription of physical therapy. No issue with training, mining equipment, mining methods or compliance.
The driver was driving too fast for conditions which was the root cause for this accident.
EE was taking stone from bucket to palletize. Stone slipped out of hands and landed on left foot.
Two employees were removing a pump from a sump. When removing the pump, one employee felt a pull in his groin and experienced pain.
Temp worker setting up modular trailer. Using a circular saw while kneeling on ground when ground gave a little and the saw kicked back & cut right index finger.
Employee was helping another Employee roll up a piece of conveyor belting approximately 124 feet long, by way of pushing the roll of belting, Employee stated he felt a sharp pain in his lower right groin.
He stepped out of the company van and didn't realize the ground outside the vehicle was uneven so when his left foot hit the ground he rolled his ankle.
Employee had been getting in and out of service truck. He went to get out of truck when his right foot slipped off running board but his left foot stayed on causing employee to hyper extend left knee. Ground was muddy.
Step off Cat 740B Haul Truck to clean back up camera and slipped on ground, twisted left knee, climbed back into cab - reported to shifter at end of shift - June 2, 2014 7:00AM
Move crew had set feeder about 30-40 min. earlier & as they were removing items from the load end of feeder the injured's foot was under the load end when it settled into the coal bottom & onto the top of his left foot which created 2 non displaced fractures that did not require immobilization. He was wearing metatarsal boots at the time which lessened severity of the injury.
Employee felt tightness in his lower back while exiting the underside of a drill.
Employee was stepping out of storage shop, stepped on pebble twisting/spraining right ankle.
Employee was using a skid steer loader to clean material from ground in finish room. As he was scraping the bucket contacted hardened material, stopping machine and throwing employee against safety bar. Employee was wearing seat belt. Employee was cutting a piece of rubber, wearing gloves, when he received a small laceration on his left thumb.
Employee was pulling scale out of #3 kiln hood when a piece of scale became stuck in the cooler grate. When this happened, employee felt a strain in his lower back.
EMPLOYEE WAS REPLACING A BELT AT THE CRUSHER AND STRAINED HIS LOWER BACK.
Employee reports backing into a stationary roof support pole while operating a forklift. The employee reported neck and shoulder pain but denied medical attention at the time of the event. The employee request a medical evaluation on 7/8/14 as the pain persisted. The employee was diagnosed with a right shoulder (rotator cuff) strain and prescribed physical therapy.
EE had went to the emergency room due to something in his eye. He found out he had a small metal piece in his eye that was removed, then cleaned.
As the employee attempted to remove cattle from the mill site, a cow ""charged"" the employee. The employee jumped off an embankment to avoid the cow and injured his heel as he landed on the roadway. Accident did not involve compliance issues, mine equipment or mining system, or miner related skills training or attitude. Required PPE was utilized at the time of injury.
Employee was using a manlift to remove a guard off of a piece of machinery. When he pulled back on the guard he stumbled and hit his right arm on the handrail.
Employee was adjusting plastic on pallet stretch hooder machine. While adjusting the plastic his left thumb got pinched between the feed roller and the tension roller.
Employee was getting on personnel carrier when he slipped and struck his lower leg against the frame of the ride.
Employee was walking down a slope to put a prism on a bench when he slipped/tripped and fell forward spraining his fingers/hand.
Employee was closing valve and was sprayed with liquor on face and arms.
EE was working on the bagging line and bumped his hand on a metal table/ledge. He went to doctor a couple days later. He stated he was okay and then started complaining that hand was hurting again and went back to doctor. EE nor doctors have let given us any diagnosis.
Finger under a thin rock as operator lowered top jaw to cut the rock did not move finger from under rock causing the middle finger on the end to be smashed. Resulting in lacerated finger requiring bandages and antibiotic cream and checks for infection.
Miner was operating a 150 ton Water Truck on a surface haul road. Hydraulic system failed. Miner lost steering assist. Miner pulled a muscle in his shoulder while attempting to regain control of the vehicle.
Employee was cleaning up stone that had spilled off a conveyor from previous loading. He slipped on loose particles that were on concrete surface and landed on shoulder and elbow. Minor scrapes occurred on arm, a large contusion formed on elbow, and soreness was reported in shoulder. Local med provider placed him on a lifting restriction. Possible rotator cuff issue.
Employee was working indoors on a Hydro Splitter and was cutting the ends off the stone to square them off. While lowering the head of the machine, the stone flipped up and flipped up pinching his left hand between the stone and top of the machine. This resulted in a left hand middle finger fracture.
Miner is a quarry Block Worker. He was ground stacking pallets when a piece of limestone slipped from his hands crushing his fourth left finger
During first responder training the employee was changing O2 bottle on SCBA in bent over position and when he stood up with this SCBA pack on his back he felt a strain in his groin.
Employee was cleaning debris from catwalk when he felt a pop in back. Then mentioned he may have caused this injury previously on June 21st at 9:00 AM employee was moving parts and supplies into the new shop addition when he was walking backwards and tripped over a pallet, falling on his back. He did not report at the time. Employee returned to work with restrictions.
Injured worker was doing general clean up sweeping, shoveling, and using a wheelbarrow. Was at home a couple hours after work. When getting out of a chair felt stiffness and shooting pain in his lower back. Reported the incident the next morning. Sent to a chiropractor. Was put on work restriction 6/25/14.
Employee was assigned to rake scale from #2 kiln. While raking a piece of scale came over the edge of kiln and hit rake he was using causing him to lose grip on rake and rake hit him on the left jaw.
An employee with 1 year and 26 weeks of mining experience was working to clear chunks from a weigh feeder, using a bar. The employee's grip on the bar slipped, and the ring finger on the right hand was pinched between the bar and the guard for the weigh feeder, creating a small laceration on underside of the second metacarpal joint.
As EE was pulling screen from screen deck he sprain/strained shoulder.
Employee was walking down the stairs when he tripped and fell back hurting both shoulders and his back. 7000-1 being done due to Part 50 audit.

Worker was attempting to close an inspection door on the pre-heater tower using a sledge hammer. He was hitting one of the hinges with the sledge and missed the hinge causing his pinky finger to be caught between the door and the handle.
EE was coming down the stairs from the second level on R-202 and reported losing his footing and feeling a ""pop"" in his right shoulder as he held on with his right hand to steady himself. EE reported as a First Aid Only case on 06/21/14 and did not seek medical evaluation until 06/27/14. He was diagnosed with a ""right shoulder strain"" and released to work with restrictions.
EMP STANDING ON 6' LADDER ATTEMPTING TO OPEN A 2"" BALL VALVE ABOVE HIS HEAD. WHILE PULLING ON VALVE HANDLE HE FELT A ""POP"" IN HIS RIGHT SHOULDER. THIS WAS CONSIDERED A TORN BICEP. HE SAW OUR PANEL ORTHOPEDIC WHO STATED HE WAS TO BE ON LIMITED DUTY FOR 1 MONTH.
The injured miner was kneeling on top of a tank truck to close the hatch. While moving from the kneeling to standing position he felt a pop and pain in his right knee. Additional diagnostics(MRI), modified work duties and physical therapy now makes this injury reportable.
Employee helping with screen change on wash tower, when placing new screen in position, employee did not move his finger and it was caught between the screen and side rail.
The man was sitting on the mantrip. As the mantrip began to move, he noticed the lunch bag on top of the mantrip was rubbing against the roof. He stated he told the operator to stop and reached for the bag. The operator did not hear him and continued to tram. The mantrip struck a rock causing the man's arm to momentarily be pinched between the mantrip and the roof.
Employee was waiting at the time clock to punch out at the end of his shift. He noticed he had left the south warehouse lights on and went to turn them off. Employee did not put his PPE back on and took a shortcut under the frame work of a walkway instead of walking around and struck his head on a beam. It required two staples to close the wound.
Employee was rodding out an Australian valve and when he got up to look at it he guesses he bumped the caustic valve that was beside it. Caustic sprayed
On 6/19/2014 the employee slipped and fell on the 2nd step of the Haul Truck causing pain in her shoulder, she was evaluated and released without restrictions. On 6/21/2014 the employee returned to the clinic and was given restrictions. On 6/26/2014, employee was released to full duty but returned to clinic on 7/1/2014 and was given restrictions.
Employee was taking the counterweight cover out of the cone crusher. When he pulled back to get it loose, his hand hit a piece of brass bushing, and it peeled away some of the skin on his hand.
While troubleshoot a cement flow problem, the employee proceeded to close a manual gate from the silo that fed a pneumatic gate. The employee unbolted an inspection cover on the flange that connected the manual gate to the pneumatic gate. Upon unbolted the cover plate, cement shot into his face due to incomplete closure of the manual gate.
EE was descending from 977 front end loader when he heard his left knee pop. EE did not report the injury until the following morning and taken for medical eval. EE was placed on modified duty for left thigh strain on 6/23/14.
Removing the drill bit from hammer to replace the blow tube using a hydraulic chain wrench, left hand was on wrench when pulling wrench with hydraulic cylinder thumb on left hand was crushed in cylinder to wrench pinning point.
Employee was loosening a screen's spring pad with a wrench. While using the wrench, he felt pain in his right elbow which got progressively worse.
The employee was attempting to bar a large rock that was stuck in a muck chute. As he pried on the rock he felt a sharp pain in his back. He was diagnosed with a back strain/sprain.

The miner was in the process of rebuilding a 300mm pump assembly. While holding a wrench in place the wrench slipped and his right pinky finger was pinched between the wrench and the flapper arm on the pump. There laceration required four stitches.
Employee was walking and tripped on a curb at a local convenience store after refueling his company vehicle. He fell on his elbows and knees bruising them and injuring his right shoulder in the process.
The following contributed to the injury: The weight of the inspection door, limited working room due to a 3"" line, lack of help from a second employee and position of the employee's hand as he pushed the inspection door back into it's original position. The employee suffered from an avulsion to the soft tissue on the tip of the right index finger.

The man had repaired the shuttle car cable. He was sitting in the shuttle car winding up the cable slack. He leaned out of car to see how much slack was left when the cable came up quickly and struck him on the head.
Employee stated that he was backing the Cat 988H out of the heading with a bucket of muck, when the units rear wheel abruptly dropped through a pothole. The operator shared that he was sitting sideways as normal for his preferred method of vision. The abrupt floor changes and jostling affected the miners lower rt. back.
Employee was walking out of the Powder Mill lunch room. When he stepped down from the porch, he felt a sharp pain in his left lower back.
While working on a pump in the maintenance shop, the EE had his hand placed on top of the pump, lining up the chuck in the shaft. Another employee had a pry bar lifting on the opening of the shaft, when the bar slipped, striking the injured employee's hand, breaking the metacarpal bone in his right hand.
Employee was changing out a coolant sensor and received 2nd degree burns on hand and arm due to contact with heated coolant.
While assembling loader tire bead band became bound. Trying to seat the band the tech struck the top portion of bead band and lower portion sprung back striking tech in the upper abdominal area.
One loader backed up and ran into another loader and employee complained of neck pain.

An employee lifted a piece of 40-lb rail with another miner. As they were walking the rail to where it was going to be installed the employee tripped on a 1"" hose that was laying in the walkway. The employee fell and the rail landed on his right shin causing a severe hematoma.
Employee smashed his finger between a sledge hammer and a table, fracturing it.
Injured was taking wire off of a bundle of roof mats. As he untwisted the wire that tied the mats together he sprained his thumb.

Employee was working indoors on the line taking stone off the conveyor and placing on a pallet. While picking a stone off the conveyor, a stone flipped over and struck the employee's left hand pinky finger. The employee was lifting a 60 lb. stone and was struck by a 10 lb. stone. His finger was pinched between the two stones.
Employee states that while moving a pallet of stacked poly bags to the stretch wrapper the bags fell off the pallet. Employee stopped the forklift and proceeded to reload the pallet with the poly bags. Employee states he had already lifted 10 bags and was on the 11th bag when he lost his footing. While regaining his footing employee felt a pain in lower back.
Employee was removing a flexible conduit connection to make a repair. While using a pipe wrench to turn a fitting, the wrench slipped and his right little finger struck the conveyor frame next to the electrical fitting. The tip end of the right little finger was fractured.
Injured employee was standing next to chute. As the chute was cut the chute moved pinching employees finger against steel beam.
Employee slipped in the parking lot while on the way to his vehicle and fell onto his right side/shoulder.
Employee was loosening bolts with an impact and wrench. Wrench slipped and hurt his middle finger. Employee didn't mention the injury until next day. X-ray shows no break but MRI has been ordered to check for tendon damage.
