Lost power on PPL mainline causing a total power outage at the underground mine rendering the hoists inoperable.

Operator was moving empty emulsion totes from the UG powder mag to the surface, operator stated they smelled a burning odor from the loader. Operator asked two other employees if they smelled it. They stated yes and inspected under the hood compartment, did not observe any visible signs of a fire. When the two employees went to leave they saw sparks/flames under the loader.

A spot was found in the SP Material Hoist rope by x-ray. The hoist was taken out of service, the rope shortened to remove the spot, tested, and placed back into service. MSHA was notified via the hotline, email to the FOS, and in person on site.

Found an unintentional roof fall at break #49 of the #2 beltline.

An unintentional roof fall occurred at HG-14 XC 110 Travelway and Primary Escapeway. This fall impeded travel in the primary escapeway from the HG-14 CM Section. The Phase 1 action plan has been submitted, approved, and implemented.

Two unintentional roof falls occurred on 24 Zone discovered by a Compliance Officer in the #9 entry. At 34 blk. intersection and between 32-33 blk. Both fell 10' high with a distance of 20' measured at the 32-33 blk. location. All approaches have been dangered off and timbered. No escapeway, equipment, or ventilation was damaged by the fall.

A non-injury roof fall that measured 12' long, 12' wide, and 8' high occurred on Spyder Panel 2, in the #4 entry at break 9. The fall was isolated to the #4 entry at the active pillar line. The release of supported roof was due to an overlying undetected separation above the anchorage of the primary support installed.

Found an unintentional roof fall in the #3 Belt line at break # 19 that is an extension of a previously reported roof fall on the #3 Belt line at break 18.

A roof sag was noticed in L27/C43. The area was barricaded off. The area fell on 7/20/21 and was found at approximately 10:50 am. by the day shift foreman.

Tower Elevator stopped about 4 foot above landing just after it started to lift, Maintenance tried to lower it but was unsuccessful, called the Elevator company out to repair. Three employees were in the elevator for longer than 30 minutes and the event was called in. Non injury

Power brown out from NYSEG power supply, they were updating their system and power to our local grid was affected. #3 skip kicked out, reset, men brought to surface, ran test trips, all tested good.

A rockfall was discovered by a weekly examiner at 1W 25xc in the left alternate escapeway. This blocked the escapeway but did not impede airflow. The mine foreman was notified and the section was pulled. MSHA was notified on site, via the hotline, and a call to the FOS.

July 18, our #1 shaft Service hoist went down at over temp approx. 6:00PM. The AC units in the control room had clogged filters. We cleaned the filters, were able to cool down the control room and the VFD's were reset by 830 PM, NO damage to the hoist or hoisting equipment, a COURTESY CALL made to MSHA Hotline. This did not affect entry/egress from the mine.

An unplanned roof fall occurred on the West Mains one break inby spad# 1653 in the No. 1 Entry. The fall was approx. 20' wide, 30'long and 14' high.

Pumps in the main portal pit could not keep up with significant rain event and allowed water to inundate into the portal openings in the mine. No one was injured and ventilation was not disturbed.

An unplanned and reportable roof fall occurred at the 177 bk #2 entry intersection in the F-Mains return. There were no injuries, no property damage and the ventilation system was not disrupted. This occurrence was immediately reported.

Pellet Plant grate/kiln stack area of the roof caught on fire. Investigation revealed a small piece of refractory had been released onto the roof when the stack cap was opened earlier in the day.

A breaker in a load center failed as designed which caused some smoke to be released. There was no indication of fire at any time and Leeville and Pete Bajo mines were evacuated as a precaution.

#1 salt production skip kicked out, cage stuck. Inspection reveled pin was missing that held safety dog to skip. Dog did deploy correctly to stop cage travel as it should have.

Lightning caused forest fire (Mud Lick Fire), Reported via MSHA hotline on 7/13/21 - fire had not advanced to mine site. US Forest Service issued evacuation order due to smoke and fire approaching mine permit boundary on 7/15/21. Follow-up MSHA Hotline Call provided on 7/15/21 due to Evac Order. No injuries, all site personnel safely evacuated.

At approximately 1:28pm an unplanned roof fall was discovered at the top of Slope B conveyor. Deterioration of the top caused the roof fall. The fall was approximately 8-10' of black shale for the length of 18'. A roof fall clean up and support plan was put in place.

The South Portal elevator was down due to a problem with the floats in the pit area at the bottom of the cage. In trouble shooting a bolt was also found broke out of a door switch. These were both repaired. MSHA was notified via the hotline. MSHA also came and inspected the elevator after it was repaired.

An unintentional roof fall occurred in 9L section. The fall occurred at 17 wall crosscut, approx halfway into the crosscut between #1 & 2 entry through #2 & in the crosscut 2-3. The fall was approx 120' long by 16' high & 16' wide. A site specific mine plan was submitted to rehab the area. *Adverse geological conditions caused the immediate strata to separate & subsequently fail.

Found an unintentional roof fall at break #17 of the #3 Track entry. Fall is approximately 19 feet wide by 60 feet long and 6-8 feet thick.

A fall of roof occurred in the #2 entry at crosscut #2 along, 2 NW Submain. The fall is approximately 20ft. long by 20ft. wide by 8ft. long.

Hot clinker dumped from the pan conveyor onto the rubber conveyor belt causing the belt to heat up and burn. This was caused due to a programming error programmed by the manufacturer of the plant when a detection of hot clinker is identified.

At approximately 5:15 am the night shift foreman discovered a roof fall underground in L27/C41. The night shift Foreman called the mine manager and coned off the area. The mine manager called the MSHA hotline. The area remained coned off until an investigation could occur.

A face ignition occurred on the 4RHG section in the #1 entry at #4 block inby spad # K2005. The continuous miner was trimming the top when a fire occurred on the mine floor. The CM was backed out 15' and the fire was extinguished by the coal crew in 5 minutes.

Two Hoist at the bottom of shaft unloaded bottom of cage and would not jog down to unload the top of the cage. Issue was investigated and issue was fixed. Hoist was operational at 7:45 a.m. MSHA was called to let them know.

Hoist was at a stop when Hoistman heard clicking noises in control room. Hoistman attempted to move the hoist with no response. The hoist began to alarm as several resets were engaged. Electricians tried to troubleshoot. Hoist consultants were able to find and correct problem. At no time was the conveyance occupied.

Surface lost power, which affected the mine. After generators were started, #2 hoist had a brake fault, which was found during checks. Miners were staged at 2 hoist. Issue on hoist was resolved and mine was evacuated. Mine still had utility power for 6 shaft. MSHA was called when issue was resolved on hoist.

#3 hoist tripped and would not reset because the west brake was not functioning properly. Through troubleshooting it was found that the west brake switch plunger was stuck in the open position.

The #7 Escape hoist had to be taken out of service due to maintenance issues.

EE is believed to have been either chipping away several inches of top layer of brownish colored granite or drilling a chain hole through the granite near end of the block on the open face so block could be lifted using a chain. Granite block being worked on was approx. 5'x12'x30'. Additional submitted details on file.

A contract employee was conducting maintenance duties at the number 6 cement cooler when the employee fell approximately 25 feet onto a concrete floor.

A Millwright drove the pins out of a 200-ton crane boom to change out a section and the boom fell on employee.

Employee was conducting ground control measures when a rock fell striking the employee while high scaling.

Employee was replacing cutting edge (approx. 8ft x 10inches x 2inches) on a 834 rubber tire CAT dozer drag box in the truck shop. When lifting the final cutting edge after the others had been installed, the load became unstable and fell. The uncontrolled movement of the cutting edge pinched/crushed the employees right thumb.

EE was removing a rock from a de-energized machine, stored tension in the v belts caused the unit lurch forward when the rock was removed. This severed the EE's fingertips on EE's right hand including the pinkie and ring finger to the distal knuckle.

The employee was examining 6 ML Belt. EE ducked under a waterline, as EE did, EE stepped in a ditch twisting right knee. The employee was scheduled off and then saw a specialist on 7/23/21. At this time EE began missing work.

Crew was in process of removing counterweights from a Grove GMK6350 crane. During this process, the crane tipped over onto its side while attempting to set two counterweights onto a trailer. This resulted in the crane cab falling onto the side of the first trailer adjacent to the crane pinning the employee's legs. Employee's injury details are described in section 7.

Fall down stairs

Employee slipped on stairs in the prep plant going down from the 10th floor to the 9th floor. When the employee slipped they grabbed the handrail and strained left shoulder and also over extended and strained right hip. Employee was wearing the proper PPE. Employee did not start missing work until 8/2/2021.

IE was checking the dumping solenoid inside the frame of an end dump. EE needed to move and was crawling out from under the truck on hands and knees. EE placed right hand near the base of a dump cylinder, unaware that the solenoid began working and the bed was coming down. EE had 3 fingertips crushed between the middle stage and the base of the cylinder.

While driving a Komatsu WA500 Loader, an employee drove over a rough road and bounced in EE's seat, injuring EE's shoulder. When EE bounced, EE caught self on the way down with EE's R arm on the arm rest, which ended up hurting EE's R shoulder. The employee said speed was not a factor.

Employee stepped through a man door and pain/pop in left knee. Employee was taken off work on 8/02/2021.

Fall from machine

I.E. was getting out of the cab of SM02 and caught EE's right foot between the seat and steering lever. I.E. was falling and used EE's right arm to catch self that is when EE felt a pop and immediate pain in EE's right shoulder. IE is waiting to see a specialist. This happen on the surface at the Akin portal, the conditions was dry and still daylight.

Fall onto or against objects

Employee tripped on a hose or rock while refueling EE's haul truck at the waste dump. Employee fell against the containment and struck EE's head, resulting in contusion.

INJURED WAS HANGING A LOOP OF CABLE FOR #4SC, THE ANCHOR FOR #3SC JERKED AND CAUGHT THE INJURED IN THE LEFT EYEBROW. CAUSING A 1 1/2"" TO 2"" LACERATION.

Over-exertion in lifting objects

IE was installing Rail Bars (secondary roof support) on the Unit 1 travel road at 14 x-cut. After the job was completed the IE mentioned to supervisor that back was sore. The IE finished the shift operating a piece of equipment.

Employee was assisting a team of 5 with moving of conveyor belts to a different area within our property. Employee climbed onto conveyor belt #2 to guide the operator with conveyor #1. While moving, Conveyor #1 got caught on a rock and swung back hitting conveyor #2, causing Employee to fall.

While repositioning a bad relay bar for removal from the face the miner disconnected the safety chain and had another miner remove the come-a-long, when they did this the relay bar slipped off the shearer sliding down the other relay bars pinning miner's right leg against a relay bar resulting in a fracture to the right tibia.

EE was getting ready to lift a chute assembly to cone crusher when the lifting chain on the forklift inadvertently caught it moving it about 1 inch pinching EE's left index fingertip to the bone.

2400-081 Mucking out ore heading hit dag or rock while digging out the pile causing an abrupt halt bouncing miners body in mucker. Dismounting mucker noticed left abdominal pain, possible hernia.

While installing outside bolt, rib rolled out mashing left foot.

Absorption of radiations, caustics, toxic and noxious substances

IP grouting cable bolts when hose came loose from tube spraying grout into IP'S face. IP did not report incident to operator until Aug 3, 2021.

EE was in the hopper working on the warrior screen. EE stepped up and out of the hopper, onto what EE thought was steel, but turned out to be rubber. EE fell approximately 6 feet onto a pile of rocks, similar to light stone fill. EE bruised EE's right leg above the knee/thigh area and scraped the back of EE's right arm.

Employee was attempting to bolt a tire back on a tractor and was stabilizing the tire with left arm, when the tire shifted and jarred shoulder.

Caught in, under or between a moving and a stationary object

While tramming feeder from #5 entry around corner into the 5 left crosscut, EE inadvertently contacted rib with one of the tram levers, while having the other tram lever engaged, causing feeder to move towards the rib pinching EE's left hand between rib and feeder, fracturing hand and fingers.

Employee was attempting to flip a rock off of the feeder when EE's ring and pinky fingers on EE's left hand got caught between the feeder and metal causing the two fingers to be crushed.

Over-exertion in lifting objects

Loading belt structure on trailer and felt pain in left shoulder. Was taken off work on 9/24/2021 for surgery.

Employee was pulling down a piece of curtain between the No.1 and No.2 entry, while pulling the curtain from the rib, a nail used to secure the curtain flew in over top of safety glasses striking them in the left eye.

EE was working under a 769C haul truck on the driveline. EE went to get out from under it and when EE went to stand EE couldn't hold EE's own weight. EE was able to get to EE's service truck and call for help. EE was driven to the ER to be checked out. The doctor believes EE has potential nerve damage or it may be sciatica.

Employee stated that while getting on the 980G Front end loader, EE slipped and twisted their neck and back. *Employee began missing work due to this injury on 8/16/2021.

Injured was returning to the quarry with unloaded Cat tractor/trailer haul truck. Injured was momentarily distracted by an unidentified object on the left side of the haul road which caused EE to drift toward the rock berm on the right side. The right rear trailer tires struck a rock which caused EE to lose control and exit the road.

Employee was assisting on a hard bar pin for dozer. Employee was helping to get the pin lined up with the eyes to fit in. Employee was observing while another employee was striking the pin with a hammer. Small piece of metal chipped off the pin and struck the employee in the left knee. Employee will have the metal surgically removed.

Absorption of radiations, caustics, toxic and noxious substances

Employee was leaving work for the day. They got into their own personal vehicle which was on the mine site, a bee followed them in at the same time, stung them on the left cheek. Employee is allergic to bees, did not have an EpiPen. Employee drove to Emergency Room for treatment. Treated and released with prescription, no work next day. Returned to work 7-29-2021.

While placing roof bolt resin in bucket liner, felt a pop and pain in left shoulder.

Caught in, under or between running or meshing objects

A roof bolt operator was injured when placing EE's drill steel into the drill box chuck. EE inadvertently hit the rotation control lever while holding the drill steel. The glove on EE's left-hand stuck to the drill steel and EE's hand/fingers were twisted around with the steel. One finger sustained a fracture, laceration and dislocation, & the other finger was dislocated

Absorption of radiations, caustics, toxic and noxious substances

Employee was plastering stopping using B-Bond. Was Wearing rubber gloves that fit up to elbows. Several days later employee developed rash due to exposure to B-Bond. Determined to be Occupational Illness on 08/13/2021.

Over-exertion in pulling or pushing objects

Mill Maintenance man was repositioning a sheet of diamond plate on saw horse - felt a shoulder muscle pull (left shoulder). Surgery to left shoulder was done on Friday, November 26, 2021

Struck against a moving object

Employee was operating a scraper, transferring bentonite and ran over a large rock on the edge of the haul road. The resulting impact from rolling over the rock jarred and strained the employees lower back.

Employee came to work and sat down to put boots on and pulled lower back when they bent over needing medical attention.

Over-exertion in lifting objects

EE was picking up 3 boxes of filters. The boxes were sitting on a piece of concrete about knee height. EE lifted all 3 boxes and felt a pain in groin area.

Three miners were team lifting a steel I-Beam. When lifting, one felt a pain in EE's lower back. EE has a pre-existing back injury. EE was diagnosed with a back strain/sprain.

While pulling a roll of mine curtain injured experienced pain in their back.

Over-exertion in lifting objects

EE was picking up the miner cable to lay across the head to back across the section. EE felt a pain in lower back. EE continued to work until 9/07/2021.

While trying to remove bearing and housing from screening plant with sledgehammer a co-worker hit IE's left thumb which resulted in 14 stitches.

Struck by falling object

Employee was bolting top on the operator side of a Fletcher Bolter. While bolting, a piece of rock fell crushing the tip of the employees fifth finger on the right hand.

Employee was assisting with manual installation of v-belt onto drive motor pulley assembly when fingers were pinched between the belt being installed and the pulley surface. Employee was not aware of hand position relative to the belt being installed. Employee had hand in an exposed position. This resulted in a crushing type injury middle and index fingers.

Over-exertion in lifting objects

Employee sought medical treatment after experiencing back pain while unloading trash. Operator notified on 8/16/21 that employee was diagnosed with pre-existing degenerative spondyloarthropathy, a form of arthritis, after results of an MRI indicate no significate changes from a previous MRI conducted on 6/2/2020.

Employee was taking a bag of trash out of the bath house facility to put in a dumpster. After throwing the bag of trash in the dumpster, EE stepped in a small hole in the gravel parking lot and twisted left ankle. Employee failed to pay attention to where was stepping. Employee didn't violate any rules or regulations.

Employee was getting out of a CAT Loader. When EE went to step onto the ground the ground was unlevel resulting in the Employee turning and twisting ankle spraining EE's ankle.

Employee was operating the right side drill of the roof bolter and as employee was positioning the drill steel and adjusting the angle of the drill box, employee's self rescuer pouch inadvertently snagged the rotation lever while employee's left hand was on the drill steel.

EE stated that as they were unloading a trailer from a tractor using a ratchet strap. EE was pulling up on the tongue with right arm and pulling on the strap with left arm, when they felt a pull or strain in right shoulder. Employee continued to work until 9/8/2021, when they had surgery for a torn rotator cuff.

Over-exertion in lifting objects

Employee was bending over to pick up a wooden crib block when they felt a pain in abdomen. EE did not begin missing work until 07/29/21. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

While unrigging bag EE fell off the trailer and injured arm.

Over-exertion in lifting objects

Employee was lifting an I-Beam from the bottom onto the overcast when EE felt pain in right shoulder. The individual worked until 7/26/2021 making the incident a lost time injury.

Operator was diagnosed with an umbilical hernia, same area of multiple previous hernias. Employee felt pain when they attempted to open the Kiln Preheat door.

Caught in, under or between a moving and a stationary object

A fully trained miner was removing a cover when the miner smashed a finger between the cover and a rock. The injury was not the result of any compliance issues nor were any protective guards involved.

A fully trained miner was walking by a supply car when they slipped and fell injuring a wrist. The injury was not the result of any compliance issues nor were any protective guards involved.

Slip and fall at ground level near the drier pad, per employee they ""tripped over our feet"". Twisted ankle and knee. Did not initially want medical evaluation, but pain grew worse and they asked to be seen on 7/26/21. No diagnosis yet, other than ""nothing broken, possible sprain/strain"". Doctor put them in a knee brace and off work until after follow up visit on 8/9/21.

Employee was getting out of the water truck and slipped due to mud on EE's shoes. Injured right shoulder when EE grabbed the handrail.

Employee was dragging a cobra drill thru a mandoor when it caught the door frame causing the employee to strain right shoulder and chest.

IE was pulling out the slider tube when the channel locks slipped causing IE to stumble. As IE slipped, heard a pop and felt a pop in left ankle. IE was brought to the surface and taken to be checked.

EE WAS DRIVING FRONT END LOADER. EE HAD TO GET OUT AND CLIMBED DOWN THE LADDER. EE MISSED THE BOTTOM STEP AND TWISTED LEFT ANKLE.

EMPLOYEE STATED THEY WERE RE-TAPING THE MINER CABLE AND AFTER TAPING THE CABLE THREW IT OVER AGAINST THE RIB AND FELT PAIN IN LOWER BACK. EMPLOYEE FINISHED THE SHIFT.

Employee was splitting rock with a hammer and chisel and missed the chisel and hit hand.

Employee alleges they were cleaning the road with the loader and they hit a hard spot in the road causing them to move forward abruptly in the loader.

Employee was swinging a sledge hammer in a coal chute and felt something pull in their shoulder.

Employee was attempting to remove a partially cut shank from a bucket via sledge hammer, a piece of the hammer chipped off lodging into employee's neck/shoulder area

Employee was unloading Dapco Tractor tire off a diesel ride and felt pain in left shoulder. Employee started missing work on 10/21/21 to have surgery on shoulder.

IE was walking and hit head on a board on the mine roof. EE then felt a pain in lower back. IE did not seek medical attention until EE's next shift.

Over-exertion in lifting objects

EE was picking a piece of rock up out of the roadway when EE heard a snap in EE's neck and felt a sharp pain.

IE was driving ""Boomer"" rings in to a longwall shield leg with a hammer. IE missed striking the ring and struck right index finger causing a laceration to EE's index finger.

Driver of Haul Truck informed me of back hurting early in the week of July 19 thru July 22. On July 20 was put on light duty. On July 21 they went to Urgent Care, doctor gave them a release for low back strain, with over the counter Tylenol and Ibuprofen and no work until the July 24. Emailed Albany Office additional statement.

2400-018W Miner scaled work area then set up bolt deck and started bolting, after a few bolts scaled area again, started bolting, rock came off rib hitting back of right boot, no cut or damage to boot, but miner had laceration to back of right ankle - two stitches.

Employee was lifting a slider ventilation tube that had a lot of mud in it and felt something pop in EE's right shoulder

EE WAS UNABLE TO STOP THE WEIGHT'S MOTION. PART OF EE'S RIGHT HAND WAS CRUSHED UNDER THE WEIGHT. EE SUSTAINED AN OPEN FRACTURE AND LACERATION OF THE END OF EE'S RIGHT RING FINGER.

EE then went on to work the entire shift without issue. EE called Supervisor at about 1215 pm on July 20, 2021 and during conversation the teammate revealed to that they slipped and fell.

While drilling with an emulsion breast drill in preparation for rib pin installation, the bit caught inside the hole, rotating the body of the drill. IE's arm was caught in the hoses of breast drill, fracturing EE's arm. Incident was not the result of a training or compliance issue.

IE pulled scoop up and got out. As EE was walking under the auxiliary fan cables they dropped striking EE's left shoulder. EE was brought out and taken to the hospital.

Ee was opening valve. EE stepped down onto hose causing the hose to unhook from the valve, and lime sprayed out, hitting EE in eyes. EE was transported to NNRH then to SLC for bilateral alkali burns to both eyes. Followed up with eye doctor on 7/19 and 7/21 and was put off work through 7/26 follow-up.

Employee was carrying a header beam for a 6' door. When EE went to set the beam down EE's hands slipped, dropping the beam. The beam contacted the mine floor then bounced on top of EE's left foot. Employee finished the shift and later sought medical attention at the hospital. EE was diagnosed with a contusion to the left foot and taken off work as a precaution.

EE has had back problems for a while, so we assume EE just re-injured the old injury by getting off the loader ladder. EE was also doing quite a bit of shoveling dirt, so we have eliminated that from EE's job responsibilities, along with requiring EE to use all steps on the loader and not jumping from the 2nd to last step.

After lifting the spilboard EE went to get off the belt. EE felt the pain in back and felt numbness in leg.

While shoveling material off of a radial stacker, the employee reported feeling pain in abdomen that continued throughout the day. After following up with the physician, EE was diagnosed with a hernia on 8/27/21. Repair surgery is scheduled for 10/14/21. EE currently has not lost any time and is not on restricted duty.

Miner was drilling when the striker bar broke inside the drifter. The miner used a hammer to try and free it, the coupler was struck with the hammer causing a sliver of metal to come off and embedded in miner's left forearm just above the wrist.

PT prescribed 3/24/22

Miner was lifting 75lb vibrators to install into vibrator brackets on side of outlet hopper on a railcar. EE placed 3 into their resected brackets but when went to lift the 4th vibrator was struck with sharp lower back pain. Railcar is 37"" height and 32"" from ground to bracket, and 16"" deep. Area was free of debris and without any trip hazards.

Employee was servicing the 6"" Multiquip pump. EE removed the filling cap and pressure from the reservoir caused hot water to escape the cap. EE received burns on the left arm, side, groin and thigh area. EE was transported to Hillcrest Medical Center in Tulsa for treatment.

At approx. 1014, a crane operator was assisting with work at B-Hill. The operator was utilizing a pry bar and their hands slipped from the bar, causing them to fall (same level) to the ground.

At approx. 1428, a tire repair employee was working on the #1 tire of the 552HT. As the employee was coming down the 4ft stairs, they felt their ankle roll on the 2nd to last step. The employee grabbed the handrail with their left hand but still impacted the ground on their right shoulder. Employee felt discomfort in the shoulder area and reported incident to area supervisor.

Assisting with a horizontal pressure leaf filter, being filled with hot water, in preparation for an acid wash, on the deck to confirm when the filter was full. The carriage bolt assembly that holds the filter head, to the filter shell, failed and sprayed the employee with hot water. Employee was treated for 1st and 2nd degree burns and given a full work release on 08/02/21.

Haul truck driver was fueling at the SLV fuel dock. When they went to disconnect, they heard a hissing sound and fuel was spraying out of the top of the fuel tank. Driver was covered in fuel and began to get a headache.

When employee arrived home, the employee noticed swelling in the right forearm area. EE was pushing bolts and felt a pop in EE's back. EE did not have any pain immediately but EE's back started hurting overnight. EE reported 7/7/21 that EE was sore and wanted to see a Dr.

IE was throwing crib ties up on a square set for a false roof and lost footing falling into a pile of rib rash. IE's hard hat came off before forehead came into contact with the rib rash. The action resulted in a laceration that required eight staples. IE was working on the mine conveyor belt that was locked out and the top belt was no more than waist high.

Employee was working on a product spout in silo 1A and attempted to manipulate the limit switch in order to make the spout go down. EE had the guard off of the electric motor and when EE reached in to manipulate the limit switch EE's left forearm had contacted the cooling fans on the rear of the motor. This laceration required 19 staples to close the wound

Employee was helping put together a belt take up unit in the 11L. As they were inserting a pin into an adjustment hole they inadvertently rested left ring finger into another adjustment hole. Employee was working at the 2250 level changing expansion joints. When they came back to the shop for lunch they began to show signs of heat stress.

Inhalation of radiations, caustics, toxic and noxious substances

Notification was received on 07/15/2021 that this retired employee has received a 10% award for Occupational Pneumoconiosis. This incident was caused by the employee's failure to wear appropriate respiratory protection during mining career. Employee did not violate any rules, regulations, or policies and has proper training. Employee retired on 5/28/2019.
