Accident type, without injuries

An unintentional roof fall was discovered on the 24 Zone beltline measuring 9'H at the intersection tapering down to 6'H in the crosscut for a distance of 50' appearing to tie in to an existing fall. No injuries to any individual occurred with this fall.

Contractor was servicing the brake pump on the hoist instead of the pump for the thruster brake. Thruster brake has its own brake pump, test trips rectified the problem. Evac called off before bringing people out of the mine.

There was an unplanned roof fall above the anchor points in intersection C38 and L37.No equipment was damaged, No miners injured. There were no miners in the mine at this time. It was discovered at time of pre-shift inspection.

During a pre-shift examination a roof fall in the intersection of X-cut 42 in the number 1 entry of 14th left longwall section MMU 006-0 was located. No miners were trapped or injured as a result of the fall.

During the Weekly examination of the left return (1st South Main) off the #3 unit, in the #1 entry between spad 16+80 thru 18+20 a Roof Fall was found in the Examiners travel entry. The fall was 18'x10x200 feet.

An unplanned roof fall was discovered in ML 4 Belt at 35 block. The fall was approximately 7-15' high x 15' wide, and extended back in the crosscut towards the intake. No one was injured during the fall.

A roof fall was found at approximately 10:15pm in the #2 track entry on the Whittaker mainline haulage from 6-9 1/4 to 6-9 1/2. The fall was approximately 35 feet long by 16 feet wide and varied from 8 to 10 feet in height.

At approximately 10:30 PM smoke was observed in the B Pit Pump house due to the overheating of a mining pump motor. The local fire department was called and, at 11:15, the smoke was cleared, and a small fire was observed inside the pump and motor. The fire was fully extinguished at approximately 11:40 PM. This occurred in an unoccupied area of the mine.

Roof fall in the south portal #3 slope. Approximately 370' from base. Roof fell to a maximum of height of 8 ft (1 foot over the anchor zone). This did not cause any change of ventilation or danger to employees. This is a very remote area of the mine.

The secondary counterweight inadvertently disconnected from the primary counterweight on the North side, which caused the South side chain to break. This allowed the secondary counterweight to fall to the bottom of the shaft and prevent operation of the hoist. The hoist and its structural components were not damaged from this occurrence.

A non-injury roof fall has occurred in the primary escapeway out by the 002/003 MMU on the #23 belt at survey station #3794, #4 entry xcut and #5 entry. Slip fell across the entry. Additional supports (cribs, jacks, and rails and jacks) were installed. Fall area measured 80' long, 20' wide and 12' in high.

A roof fall was found at approximately 8:30am in the #6 entry on the Old Main Line Haulage off of Whittaker bottom between the spad numbers of 336+00 to 337+00. The fall was approximately 100 feet in length by 16 feet wide and varied in height from 8 to 10 feet.

No. 3 Hoist - 4 power bumps then AVISTA restored power. At approximately 1910 hrs. hoist kicked out - loop fault breaker fault. Electricians responded - removed damaged loop fault breaker and installed new one into system.

The wiggins fueling valve on the south side pad of the 5280 Lube dock was not maintained in a safe manner. It didn't shut or close causing the #666 to catch fire.

Fall of ground in the 917-100 level that was in the process of the bolting cycle. Bolting had not yet been completed when the back failed, partially covering the front of the equipment bolting the back and ribs. No injuries to miners working in the area or from this event. Contributing factors under investigation.

The roof fall was encountered around 6:30 pm on 08/16/2021. The roof fall area is located at crosscut #79 on 1 East Mains, #3 Belt in the intersection. The roof fall is approximately 20' in length in the intersection on the beltline and extends approximately 35 feet into the 4 left crosscut and ranges from 2' to 6' thick.

An unintentional Roof fall occurred on the S1J at the #2XC. The fall was approximately 30-foot long by 30-foot wide and over 10-foot high. The fall was discovered by the examiner.

On 08-13-2021, miners were cutting in the HG8 Long Wall number one entry, 25xc to 26xc area, a hot spot was created that could not be extinguished from underground. The mine is presently idle and the area atmosphere is being monitored from the surface and nitrogen is being pumped from the surface as well.

Found an unintentional roof fall between x-cuts 29 and 30 of the Hall Mains Beltline.

An unintentional roof fall occurred in the H1 Tailgate Continuous Miner section in the #3 return entry located at 49.5 wall intersection. The fall measured approximately 6'1' high x 8' wide x 30' long. Adverse geological conditions contributed to the failure. There were no injuries as a result of this incident.

An excavator operator was backing up a haul road and repairing the roadway as they traveled. The excavator traveled underneath a 69KV powerline owned by PPL. Unknown to the operator, the powerline arced to the boom of the excavator which cause a brief power interruption. The operator was only made aware of this at 11:30 am when an electrician showed up on site to investigate.

Test trip was being done, track limit exceeded, NIDEC was on site to diagnose, troubleshoot and change out motor encoder and adjust parameters on drive, #3 hoist back up and running within 1.5 hours

Hoist tripped out on ""under voltage"" alarm and would not reset. Hoist down for 5 hours, while troubleshooting. Broken wire on PLC due to over torque on a termination screw. No miners were on the cage at the time.

At approximately 8:40 am, Monday August 9th, 2021, a 789 haul truck (Unit#522) approached the PAD 4 ROM rock stockpile used to feed the Pad 4 crusher. While positioning the haul truck to dump in reverse, the haul truck operator stopped at the dump edge preparing to dump when the dump floor gave away causing the haul truck to tip.

After the Silver Shaft Hoist was idle for a lengthy time the weight brake would not lift fast enough or high enough to release. A consultant was called in to help diagnose the problem and identified that the brake set regulator pressure was not high enough to engage the brake boost solenoid. The regulator pressure was turned up correcting the problem.

Unplanned roof fall at crosscut 49 1/2 #4 entry, station #6414.5 on the Main North Belt line. Fall occurred in a pillar line supported with 8' T.R. bolts and 11' cable bolts. Fall dimensions 18 ft. wide, 22 ft. long, 9 ft. high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed on 03/22/2015. Reported to MSHA @ 8:32 PM on 08/11/2021.

The NP Hoist was found to have a fault from the rope slip sensor. The sensor was checked and not working. The hoist was taken out of service until the sensor could be changed. The sensor was changed, the hoist was inspected, and placed back into service. MSHA was notified on site and via the hotline.

A reportable Roof fall occurred in the I-5 #1 entry at crosscut 26. The roof fall fell above the anchored support and MSHA was notified.

A roof fall occurred sometime the weekend of 08/08/2021. When investigated moisture was discovered in the roof at area of the fall. Also the area was entering into a hollow with lower roof cover from the mine to the surface.

At 8:20 pm section foreman encountered a roof fall outby the MMU 005 in No. 5 Return Air Course Entry between x-cut No. 28 and x-cut 27 on the DS-2 Mains where the No. 5 entry of the DS-7 Panel was started.

At approximately 11PM in the #6 entry on the west mains section we had an unintentional roof fall. This material fell in on the #3 MRS and was approximately 15'x10'x7'. We developed an MSHA & State acceptable recovery plan and retrieved the MRS safely.

The roof fall happened at the face area on MMU 003, crosscut #17, in the #3 entry. The fall was directly outby the MRS machines and was approximately 40' long 6' thick and 12' wide. The reason for the fall is unknown.

While doing EE's hoist tests on the silver shaft hoist weight brakes the hoistman could not get the weight brake to come back up. Mechanics diagnosed that an air regulator for the brakes was not working correctly. The mechanics changed the regulator correcting the issue.

The silver shaft hoist would not clutch in or out on the north side. Mechanics diagnosed that the weight on the hoist rope from pulling muck for a full shift caused the clutch to stick. The Mechanics chaired the cage on top station after emptying the skip to get any residual weight off of the clutch so it would clutch in and out.

The south brake controller on the Silver Shaft Hoist was tripped and would not reset. Electricians diagnosed a failed UPS unit and replaced the unit.

The elevator at Patterson Creek portal was out of service due to a short power outage which caused the safety relay bar/switch on top of the elevator to activate. The breaker and switch were reset and the elevator returned to service. The miner onboard the elevator exited safely once it returned to service.

An unintentional roof fall was discovered by a fire boss. The fall measured 53'L X 5-6'H. All approaches to the fall have been timbered and dangered off. No escapeway, ventilation or equipment was damaged by the fall.

Hoisted personnel on Cage #2 at Shaft #1 and struck closed vent doors causing over torque trip of conveyance. Hoist reset and lowered personnel to 1550 station to offload. Lowered conveyance #1 to transport personnel to Shaft #1 collar. Conveyance #1 tripped 175' up from 1550. After 36 minutes troubleshoot, miners were lowered to 1550 and vacated via shaft #2

At approximately 4pm the hoist man received a call from the work deck to bump up on #2 work deck winch. The hoist man reportedly heard a noise coming from #2 work deck winch, EE checked the winch and seen that the 1"" cable was unspooling from the drum and falling into the shaft. The cable completely unspooled to the work deck below due to mechanical failure of coupler.

Water truck was traveling down a grade fully loaded when the operator noticed flames coming from the right rear wheels. Operator safely stopped the truck and exited the cab after initiating a Mayday call.

Miners were taking the K2 elevator to 7th floor. accidently pressing th Estop button. The elevator stop and was stuck. After it reset itself the car began moving releasing the trapped miners. No one was hurt.

3 employees trapped in freight elevator on preheater tower between floors for 3 hours. No injuries and the condition of the employees was monitored via radio communication at all times.

A severe rainstorm resulted in a flash flood out of the canyon to the northeast of the portal, which resulted in water in the intake entry. The return entry was not affected by the water. There was no one in the mine at the time of the incident. *The miners traveling the road were involved in the flood.

Driver was at parked truck (#5 lot) to start pre-shift inspection. Truck started to move. Driver fell under the truck and truck ran over driver's legs from about the knee area down.

Struck by powered moving object

An employee was fatally injured when EE was struck by a third-party truck driver who failed to stop at a posted, lighted, STOP sign and made a sudden turn off the primary travel way. Local law enforcement arrested the third-party truck driver and charged the driver in connection with the accident.

Drowning

Mantrip was passing the loadout and they observed a wall of water and debris coming towards them. They began backing up and hit the berm. EE was the first to exit the mantrip. They were on high ground with water flowing on both sides of them when EE stepped into the stream on the east side of them when EE was swept away. EE's body was found 6 miles downstream.

EE was pulling pipe at 4050 LVL N underground. EE removed the clamp from the pipe EE was removing, and the pipe next to it fell. EE's left pinky finger caught between the handrail and the pipe. Seen at ER, diagnosed with a fracture to the left pinky, released to full duty.

Struck by flying object

EE had just cut out at the tail of the 2W L/W and was heading back towards the head. The EE looked at the tail operator, and when they turned back a rock flew off the drum and struck them on the left side of their face. EE received three layers of stitches, and a facial fracture.

EE was reinstalling the tail pulley guard on 46 belt when the guard shifted and smashed EE's right thumb between guard and a concrete pillar. This resulted in the amputation of the top 1/8 inch bone on that thumb.

Pulling thumb pin on 245G Excavator. Due to unexpected movement of thumb, miner received injury to L index finger. Resulted in amputation of finger.

During installation of 4"" pipe, a worker was injured when pipe slipped off the basket and pinched the 3rd finger on left hand resulting in finger having to be medically amputated.

Caught in, under or between running or meshing objects

An employee was helping another mag drill holes. The plate was cleaned and drill clear, another employee turned it on at the same time IE reached back in to move a rag. It became entangled in the bit and around IE's glove. This caused lacerations to ring finger and lacerations/fractures to the tip of middle finger, resulting in the loss of 1/2 inch of the tip of middle finger.

While trying to align bolt hole with a bar on a take-up frame and ram frame on a belt drive, mine employee pinched right pinky finger between bar and ram frame. Mine employee broke tip of the top bone of the right pinky finger.

A fully trained miner was putting on boots when the miner cut a toe on a ""J"" hook. The injury was not the result of any compliance issues nor were any protective guards involved. NOTE: Employee did not begin missing work until 9/8/2021.

Struck against a moving object

IE was operating a skid steer on the refuse pile. IE was traveling from the plant to the refuse pile. The skid steer slid into a dozer rut and caused a jarring action injuring back.

Employee was placing spreaders on bottom forms when the one they were standing on twisted causing them to fall four feet.

Employee was operating a mantrip on the track when employee struck another piece of track equipment. The mantrip operator was transported to the hospital.

Over-exertion in lifting objects

Employee overexerted and strained lower back when EE attempted to lift a transformer onto a scoop. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Struck by falling object

Employee was walking alongside the loader and EE was struck on the right shoulder and wrist with rocks that had fallen from the roof. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Over-exertion in lifting objects

Employee was lifting high voltage cable overhead in an outby track haulage area when EE felt pain and burning in the left shoulder. Employee is off work awaiting an MRI to determine the extent of the injury. No rules or regulations were violated and employee was wearing required PPE.

Employee was cleaning the inside of a ""CHUTE"" when EE began to feel pain in the left side of EE's back and continuing down EE's L/hip & L/leg.

Employee was receiving a piece of belt structure (bed rail) from across the DMU when the bed rail began spinning in the coal. The bed rail then wedged against the coal rib and the bed rail then started spinning against the employee's fore arm causing a laceration. Employee was wearing the proper PPE.

Employee (foreman) was on a personnel carrier while by self. EE said EE backed up and struck EE's head on a brow causing a sore neck. EE finished the shift and didn't want medical attention. EE didn't go to the doctor until weeks after the accident.

WHILE PERFORMING MAINTENANCE ACTIVITIES ON AN HP 400 CONE CRUSHER, TEAM MEMBER WAS REMOVING PIN FROM HYDRAULIC CYLINDER ON AN HP 400. WHEN PIN WAS REMOVED, THE HYDRAULIC CYLINDER CAME DOWN PINCHING LEFT PINKY FINGER BETWEEN CYLINDER AND OBJECT USED TO REMOVE PIN. FINGER REQUIRED STITCHES.

Employee was conducting post blast inspection, wetting down a muck pile. Employee tripped and fell injuring left shoulder. EE was seen by site medic, referred to ER and then to Occ Health where EE was given restrictions.

IE was walking in an entry of the mine. They stepped in a hole and twisted a knee. IE chose to seek medical attention on 8-30-21.

Struck by flying object

The employee was using a 4 pound sledge hammer to drive a boom pin out of a continuous miner, the miner removed glasses after they fogged up and begin to drive the pin out when a piece of metal came off the pin and embedded into right eye.

Struck against a moving object

A crane overturned in the sand plant area trying to overturn an industrial dust collection, the miner hit hard inside the cabin of the crane, losing consciousness for a few minutes. The miner was treated by paramedics and taken to the hospital, hours later miner was discharged because miner did not show lesions on the X-rays.

Employee changing a coal truck tire in the office parking lot slipped on a rock and popped right knee out of socket.

Employee working within subframe of the unfinished belt drive for 7 North #2 belt at 50 block. When employee stepped over the subframe employee's right foot landed on a bag of sack crete and employee rolled right ankle. The employee was wearing required PPE and did not violate any rules or procedures.

Getting down from the back of the truck and EE's foot slipped off the back of the bumper causing EE to fall on their left shoulder.

Dresco pipe ruptured, whipped to the side and struck employee in the leg. Left lower leg fracture.

Over-exertion in lifting objects

Injured was cleaning walkways along the M8 belt line and bent over to pick up rail and strained lower back. Injured was taken for observation and placed on restrictions for a lower lumbar strain.

The employee was repairing a broken screw flight shaft. LOTOTO was not implemented for this task by the employee.

Employee was climbing up the ladder of the acid trailer to unhook the airline in the crows nest after unloading. EE lost balance as EE was climbing up the ladder and slipped while trying to grab the bar/railing. EE did not fall all the way down the ladder but caught self on the ladder while falling causing strain on EE's body.

While climbing a fixed ladder mounted to the side of the sand box, employee lost footing and slipped and fell down the ladder. Employee caught self with both hands on the bottom step resulting in hypertension of hands at the wrist joint. Employee's left thumb also caught the bottom step and was hyperextended as well. This resulted in a hand/thumb injury.

A fully trained miner was climbing on a rock duster when they sprained their foot. The accident was not the result of any compliance issues nor were any protective guards involved.

Employee got off motor to throw switch and slipped in the mud, striking left shoulder on the ground. Employee worked until 8/31/2021.

Over-exertion in lifting objects

Employee stated that EE pulled Groin Muscle while removing Rock from Vessel on the 4th Floor.

Struck against stationary object

The operator was traveling back the return on the scoop and ran over a piece of omega block causing EE to roof out and cut EE's right ear. This resulted in sutures.

EE picked up a battery charger and felt pain in EE's left shoulder area. EE was seen by medical provider on 9/28/2021 following 33 days of self-care. Pending MRI

The EE was coming down from SC43, un-chocking it, EE's feet slipped out from under the shaft causing EE to remain standing in an awkward standing position straining left hip and upper leg. EE reported on 8/27 and was evaluated by company Doctor. Doctor gave EE full duty RTW. EE scheduled their own Dr visit on 8/30, WC ended up taking it. First day missed 8/31/21.

Employee was hanging 10"" waterline when the waterline came out of the chain holding it and struck employee in the right knee. It was determined through MRI results on 9-22-2021 that employee has a partially torn ACL and required therapy.

The skid, where driver was to unload, was full of standing/green water due to rain. Driver reported standing water to safety pro and the site safety team. EE was told to go ahead and unload. EE received an acid burn on their foot due to the water seeping through a hole in EE's acid boot.

Over-exertion in pulling or pushing objects

Employee was pulling on miner cable felt pain in abdomen, did continue to work from 08/25/21 until 7-25-22 when EE had to have the place repaired due to increased pain in that area.

IE was operating a mini trac to get a pallet of block. IE scooped up a loose cable bolt as well. The loose cable bolt end swung around and struck IE's left forearm. This caused a laceration requiring stitches.

EE was stung by a bee exiting EE's work truck. EE had an allergic reaction and had to go to the ER. EE received medication to stop the reaction and was removed from work for 1 day due to the side effect of drowsiness.

Employee was rock dusting along the beltline. As employee was backing up employee tripped over a loop of dust hose and fell backwards on back. Employee sustained a bruised and strained back.

While roof bolting, a small piece of mine roof struck the employee on the right shoulder. Employee continued working until they started missing work on January 19, 2022 to have surgery.

Went to 3&4 baghouse to start the lime pump. Employee called for assistance when employee wasn't successful in starting the lime flow. The supervisor came in to see there was no lime flow and the pump was not energized. The supervisor started the pump. The helper was standing at the pressure end of the pump, the hose failed, spraying the helper with lime.

Struck by falling object

As the belt was being cut for a belt move, the bolt holding the bedrail in place fell out, causing the bedrail to strike the employee in the face and neck area. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

While transitioning from kneeling to standing felt a pop in right knee. Individual had surgery for this on 1/26/2022 making this a lost time injury.

Employee was exiting 627G Scraper when they missed a step while descending. Employee fell backwards and landed on their lower back area and hips.

Employee got crib mix in boot and received burns on top of EE's foot.

IE was operating a bolter. IE rested drill steel on foot and proceeded to drop the drill pod. The drill pod struck the steel and smashed EE's foot causing a contusion. (X-rays Negative)

Miner was closing air-lock doors when head became pinched between the doors resulting in a laceration.

While assisting in the removal of an existing 4 conductor 6 gauge SO cord, the employee slipped and fell when walking on an embankment.

Employee was foaming sides of overcast, when EE's foot slipped on loose material causing EE to fall on the left shoulder. Employee started missing work on 12/7/21. EE is wanting on an MRI.

A rock jam occurred at 5 shield and employee was holding the rock breaker as another employee functioned the shield. They attempted to lower the shield 3 times and it did not drop. Tip down was hit, causing the shield to drop and pin employee between the shield and the rock. Compliance with rules/regs, PPE mining equip/systems not a factor. Miner proficiency may be a factor.

Over-exertion in lifting objects

I.E. was loading 6' roof bolts from the scoop bucket onto the roof bolter. As the I.E. lifted a handful of bolts to place on the roof bolter they felt a pop in their right shoulder along with a burning sensation. Upon further investigation the I.E. has a tear in their rotator cuff and is currently off work.

Struck by falling object

Was operating bolt machine when rock fell striking employee in lower back. Was taken to hospital and checked, bruised, wrote off work until 8/30/2021.

A fully trained miner was getting out of a mantrip when EE struck head on the roof. The injury was not the result of any compliance issues nor were any protective guards involved.

Stepped out of cab to get off of machine that EE was measuring. Turned to position self to step down backwards & grab grab-iron. This is the motion that tweaked EE's back. EE was in the parking lot not in the mine itself. EE has prior back issues from past injuries.

EE stood on the front end loader to tighten a bolt for the self leveler & landed on tailbone.

Employee was stepping down off the back of a service truck when employee slipped and fell to the ground. Causing a wrist fracture. It was down pouring rain at 7:00am when the incident occurred.

Employee was assisting with removing the jack pipes from the unit tail piece during a belt advancement. As employee pulled the pipe out of the rib notch, employee felt pain in groin. After medical evaluation it was determined that employee has a hernia that does not require medical treatment at present. This incident occurred on 8/18/2021.

Employee claims to have been shocked after replacing a battery lid on a scoop and allegedly contacting the rubber over the battery terminal. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was returning from the upper levels on Tower 3 when EE fell because the steps moved.

Employee reported an unwitnessed incident on 8/17/2021. When removing the right front wheel chock, the employee felt a pain in right shoulder. EE completed shift and went to the Emergency Room on 8/29/2021 and was placed off work. The company investigated due to the time elapsed from the incident to diagnosis and received medical information 9/8/2021.

Down on EE's knees pulling fly pad off the curtain board when EE felt a pop in EE's chest, EE finished EE's shift then went to hospital next day, was diagnosed with strained chest muscles told to remain off work at least two weeks

IE was loading supplies onto EE's bolter and strained EE's abdomen on the left side. IE received medical treatment on 10.21.2021 and was removed from work for hernia surgery.

The injured employee was rib pinning in the 9 North Mains 3 to 5 crosscut. The injured pried down a rib that was approx. 5' high 6"" thick. As the rib fell it broke and a piece struck the employee in left knee. Employee began missing work 08/18/2021.

Employee had scratched cornea from fine material in right eye due to windy conditions.

Employee was raising a head roller with a pipe wrench and it slipped off causing EE to fall against the rib resulting in EE's lower back being hurt. EE continued to work until 07/14/2022 when EE went off work for surgery.

Miner was operating EE's personal vehicle, traveling from the mine entrance to our contractor shop when the vehicle EE was operating made contact with a concrete barrier used to post a stop sign for the inbound traffic. Miner suffered muscle strains due to the impact of EE's vehicle and the barrier.

EE got the bucket to release but was standing a little too close to the machine bucket. When it released, the teeth on the bucket scraped EE's left leg in the shin area.

While changing the screen panels, the employee got hurt with a tube and when EE lowered head EE's right eye got caught on a loose wire from the Screener.

IE was assisting with a power move on the North main section pull back from HG-12. IE was pulling cable slack under the belt running to the intake stopping. When IE pulled the cable, IE felt a sharp pain in IE's lower right stomach (Hernia). IE was taken off work on 8/16/2021.

At the start of shift August 14th, employee reported having back pain as a result of hitting a hole on the road to the crusher the previous shift. It had been raining on and off all day. Employee was taken to the ER, and given a full release for 8/17/21.

A miner was injured when a chunk of coal, approximately 3' tall by 10' long & 6' thick, slid vertically down the rib and tipped forward striking miner's left leg and knocking miner down. Miner sustained a fracture to left leg.

Employee came to work 08/14/21 and reported having back pain as result of hitting a hole in the road on the 44-shovel run, near the entrance, by the spanners, the previous shift. Employee was taken to the ER and given a full release for 08/17/21.

Employee was using an electric pump to fill a sprayer with Mild Acid Detergent. The sprayer overfilled causing the hose to pop out of the sprayer. It sprayed the employee directly in the face. EE then began to flush eyes at the eyewash station. Employee was medically evaluated, treated at emergency room and was released to return to work on 8/16/2021.

Contact with hot objects or substances

I.E. was lancing the off side shear jack pin on a continuous miner. While adjusting to cut a better hole they had some hot slag and flash blow back on them. This caused small burns to both their biceps and below the neck. I.E. worked until a doctor took them off for the burns to heal on 8-16-2021.

Employee was shoveling the 2-A tailpiece placing material on a 2-A belt. Employee stated the shovel broke causing a sharp pain in the lower right portion of employee's back & right leg.

Performing maintenance on catwalk of a shaker/screen that was broken down. EE walked under a belt & must have struck the shaker. A 4-6in rock somehow shifted & came down, striking EE's hard hat, then bouncing onto EE's glasses & nose. We just got confirmation of a fracture.

EE was attempting to use a garden hose along the building. While trying to untangle a portion of the hose EE walked into the corner of a crank out window that was open to allow air into the shop. EE had a small laceration to the head, as an abundance of caution the ambulance was called. EE was transported to the hospital to be evaluated and was released approx 2 hours later.

EE pinched EE's left pinky finger between two stones while moving them.

Employee had exited equipment to speak with other operator and while walking, slipped on loose gravel/uneven terrain and fell to the ground.

Truck Driver was attempting to dump load of coal when the bed of the truck broke loose from frame and during this occurrence it caused the cab of the truck to bounce pretty hard resulting in driver fracturing three ribs on left side

IE was picking up 2 cans of oil on the head drive when IE felt a pop in lower back. IE finished shift and requested to see a Dr. on next shift.

On 8-9-2021 an employee was moving items in a storage shed on surface and a bug bit them on the right leg. The employee did not report to work on the following day due to this occurrence. The employee was released to full duty on 8/11/2021.

Employee was unloading stone slabs with overhead crane when a slab fell over and pinned EE to the ground. Co-workers rushed over to lift slab allowing employee to move. Employee was carried to safety then transported by ambulance to hospital.

Employee was walking over a small pile of rocks and some slid out from under employee's foot causing it to bend sideways and pop. Ankle was x-rayed and did not show any broken bones. It was wrapped, employee was given crutches and medication for pain and swelling.

Employee was rambolting a discharge flange on a 12"" pit pump - employee was tightening up bolts with an impact in right hand and a combination wrench in left hand & while applying pressure to the bolt employee heard a pop and shoulder began to hurt

Employee was checking on a pump in the Right Return at 30 Brk. The employee twisted a knee causing employee to fall. At the time of the injury the employee received an exam from an emt. Employee then returned to work with no lost time until 11/16/2021, when surgery was performed.

An employee was in the process of shoveling some spilled salt beneath a conveyor belt transfer. While pushing a shovel through the salt, they felt a sharp pain in their lower back. The pain made it difficult to maneuver and the employee decided they needed to seek medical attention.

Employee was using a bottle jack to lift a water pump and felt something on the elbow. EE looked and found a large bump on the end of it. It was later diagnosed as bursitis.

Injured was bending over and putting waterline together and when EE stood up said EE felt a pop in EE's knee. Injured continued working and on 8/27/21 injured reported the incident and was set up for doctor appointment on 8/30/21. On this date injured was placed on restrictions and is awaiting an MRI.

On 8/27/21, WC carrier notified by email that our employee claimed a work related injury. EE slipped, but did not fall, while on the sample circuit, causing pain to EE's left knee that EE had injured during personal activity in 2016. Currently ONGOING.

Miner stepped over to one of the tires and it rotated causing miner to start to fall. Miner grabbed the support leg of the conveyer to stop the fall and strained right shoulder and upper arm. Miner was placed on restricted duty by medical.

The Injured employee had ring finger on left hand smashed between 2 small rocks on the rollers leading to the rock splitter. The injured employee was sent to the clinic where they received 2 stitches, and on the following day underwent outpatient surgery to repair two ligaments in the finger. The injured employee is awaiting a doctors return to work orders.

Employee was scraping a transfer chute with a long-handled scraper and hyper-extended shoulder. Employee continued to work normal duties until receiving an MRI and determining that the employee had a torn rotator cuff. Employee worked until surgery date of 9/10/2021.

EE reported, while changing a bearing on the 24 drive elevator in the Kiln area that they were lifting gear housing into position when an aerosol can with Knocker Loose fell from top of the drive elevator where it had been placed. When aerosol can fell they claimed that it hit floor, then ruptured and struck their calf and thigh, resulting in a scrape and bruising.

The drum had been washed prior to the cutting but was not washed out well enough and enough time for the fumes to evacuate the drum.

The employee was installing a speed bump hammering on a pickaxe when EE felt an impact on left hand. A small piece of metal chipped off and hit EE's hand.

Employees were lowering a bolt template to the ground with a reach forklift and a hoisting bar. The hoisting bar slid off the forks because the locking pins were not in position. The template or the hoisting attachment struck an employee nearby. The pins were not in place because an employee earlier had installed it and the operator did not include that in the pre-task inspection.

Employees were reinstalling the belly pan on the undercarriage of the dozer. The rigging on the belly pan slipped allowing it to swing back down and pin the mechanic. The mechanic was diagnosed with three fractured ribs. Mechanic received a restricted work designation to lift no more than 20 lbs. and is due back to the doctor on 8/11/2021 to follow up.

EE sustained a contusion to the right forearm and minor swelling when the drill rod fell from above striking EE after the yoke cotter pin broke connecting the mainline and the rod while laying the rod on the rod rack.

Employee was going to wash off grease fittings. The employee was having an issue with the hose, so the operator was going to dismount the loader to help. The operator did not realize the loader was still in gear and as operator stepped off the brake pedal, the loader moved forward. The front right tire bumped the employee's knee.

When the end of the plate was cut loose from the crusher housing, the plate rapidly released this tension like a spring and swung out impacting the mechanic. A miner was bringing screens from the warehouse to the screening building using a forklift. While driving over uneven surfaces the screens shifted on the forks and slid onto the ground. Orthopedic Doctor recommended the replacement of both knees due to excessive amounts of bending, kneeling & walking IE has experienced working underground. The IE continued to work until 11/29/21 where IE underwent surgery to replace left knee. Right knee replacement will be scheduled later.

Replacing broken steps on a staircase, outside, on a very hot day. Employee felt overheated. Ambulance had to be called, treated for heat exhaustion with fluids, released from hospital later that day.

MINER WORKED ON AN AIRSLIDE FOR 2.5-3 HOURS UNDER OUR FINISH SILO'S AND DID NOT TAKE A BREAK TO COOL OFF OR GET SOME WATER. OUTSIDE TEMPERATURE WAS WELL OVER 100 DEGREES FAHRENHEIT.

Employee felt discomfort in the top of right forearm while sorting and repacking stone into crates. The pain developed over time and was not one specific incident. The Dr. says there is a strain of the extensor muscle, Facia and Tendon at the Forearm level of the Right Arm.
