Accident type, without injuries

At approximately 11:55pm the night shift foreman discovered a roof fall underground in R1C14. The night shift foreman called mine manager and coned off the area. The mine manager called the MSHA hotline. The area remained coned off until an investigation could occur.

While preparing to replace patch on acid storage tank, a rapid energy release out of the vent line caused the line to burst, causing unexpected release of acid to spray through the deaerator on top of the tank. During the action of grinding the sparks were going towards the overflow pipe which caused an ignition due to the high amounts of hydrogen.

An unintentional roof fall occurred in the HG-13 Longwall Belt/Escapeway, Entry 1, XC-35. This fall impeded egress from the LW Face to the secondary escapeway. The action plan has been submitted, approved, and implemented.

Weathering contributed to a roof fall inby spad#17 of the #3 entry (slope belt). The fall is 10 feet long and 6 feet high. No one was injured, ventilation was not affected, and the secondary escapeway was re-routed.

An unplanned and reportable roof fall occurred at the #35 break, #2 entry intersection on the 35 Butt section intake. There were no injuries, no property damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

On 06/26/2021, an unplanned roof fall occurred on the HG9 travel way (primary escape way)at xc72, entry number 2. The fall was contained to the intersection and about 10 to 12 feet in height. The fall appeared to be black shell rock, and had not been glued.

Production Hoist #3 went down on a limit error and shows parked. Issues were resolved. MSHA was called when hoist was came back operational at 12:30.

At Approximately 2:00 am on 6/23/2021 a fire was reported in the hoist drive transformer room on the surface located in the hoist building. It could not be determined if the fire was out, so the MSHA Hotline was called, shortly after a 103 K Order was issued to the mine, once fire was confirmed out, area was barricaded and guarded until a representative from MSHA arrived.

At approximately 10:58 AM the Prep Plant belts were shut down for a maintenance outage. At approximately 11:30 AM, this belt was observed to be on fire approximately 170' from the head roller and was extinguished at approximately 12:00 PM. Through investigation, a hot roller was presumed to cause the conveyor belt to ignite.

Dayshift miners cut out wire to perform rehab of the area. Nightshift miners were to continue this work but during the end of dayshift blasts it is assumed the fall of ground occurred. The fall was discovered and reported on nightshift of 6/18/21.

A ROOF FALL OCCURRED IN THE 4 NORTH 4 PANEL IN THE RETURN OUTBY THE SECTION. THE FALL OCCURRED AT #30 BLOCK APPX 1 X-CUT INBY SS'S 9430 AND 9432. THE FALL CLOSED INTERSECTION OF THE #2 ENTRY AND FELL IN A TRIANGULAR SHAPE ACROSS THE INTERSECTION OF THE #4 ENTRY. HEIGHT IS UNKNOWN. THE AREA HAS APPROXIMATELY 700' OF COVER AND IS IN CLOSE PROXIMITY TO A STREAM BED.

A small fire occurred at the Reclaim Pumphouse. The fire occurred outside the building when insulation started on fire near some electrical connections between the building and a transformer. The electrical department was able to shut off power allowing the fire department to extinguish the fire. MSHA was notified at 6:02 am

An unplanned roof fall occurred inby spad #1660 in the No. 2 Entry of the South Mains. The fall was approx. 15' high 20' in length and 20' wide.

Drive belts on Metso vibratory screen were improperly tensioned, which either caused or was caused by the screen being overloaded. This caused the belts to slip and catch on fire, which ignited the screen dust cover. Damage was limited to drive belts and dust cover.

A coal shovel caught fire following an electrical fault. The fire originated in the reactor cabinet area and burned for over the 30-minute time frame.

Grass fire was started when a 230,000 volt transmission line fell to the ground in the highway 314 right of way. The fire spread to mine property in the pond 24 pit 16 area of the mine. Local fire departments and mine personnel contained the fire. Fire was extinguished. No injuries or damage to property.

A brush fire occurred just outside of the Collom pit. Outside resources were called to assist with controlling the fire. No structures or equipment were threatened by the fire and no one was injured. The cause of the fire is still under investigation.

At 9:33 AM, the secondary escape way was taken out of service due to damage to the computer screen plug located on the boom truck. Production in the mine was stopped and all miners were transported safely to the surface by means of the slope hoist man car. The hoist was put back into service about 2:45 PM on 6/16/21.

Grass fire along Highway 314 spread from highway right of way on to mine property in the B/C section of pit 11. Fire was contained with local fire departments and mine personnel and equipment. No injuries. Highway right of way fence posts burned. Unknown how fire started on the highway right of way.

Roof fall was found in primary escapeway in 7RT Bleeder. Fall was not above the anchor zone. Bolts broke at 3 foot level. It did not affect ventilation. Size was 17 feet X 17 feet X 3 foot high

The Mine was experiencing high levels of H2S gas and a inundation of water at HP7 and 96 (containment site) as well as HP5 off 86 (sump) and other areas of the mine downwind of these locations. The Miners were instructed that these areas were restricted.

Mine manager was conducting pre-shift inspection when EE noticed a roof fall in one of the rooms. EE was able to stop any miners from entering the mine and called the field office supervisor and notified. There was no injuries to miners, and no equipment damage. The fall seemingly, occurred overnight when no one was working.

A roof fall measuring 18' wide by 20' long by up to 8' thick occurred at xcut 135-136 on the 2N/3ME submain entry #5. No one was injured and ventilation was not disturbed.

Two Lakehead employees were trapped in the west, Step 1, 2 Agglomerator elevator. It was first noted that the contractors were in the elevator at 10:20am. After an unsuccessful attempt to reset the elevator, the elevator repair company was contacted and the two contract employees were freed at 11:03am

An unplanned Roof Fall occurred at an unknown time in the West Mains Section between XC 17 & 18 in the #6 Return Entry. The cave is approximately 18.5 ft. wide by 30 ft. long and 8 ft. high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; Strata failure due to seam transition area.

Slid off a sluff bank and turned over dozer. Material kept the dozer in almost like slow motion. No injury windows came out whole, and back one stayed in.

The mine experienced greater than 1% methane at the top of the exhaust shaft. This was due to methane liberation within the longwall section while mining through a cross cut and the top not caving as it should following a cut. This led to an evacuation of the mine and changes to the ventilation system to reduce the levels to below 1%.

Employee was working on preheater tower, completed the job and went down elevator. Elevator stopped working while miner was inside at 1805. Doors would not open and elevator would not move. An electrician was able to get the elevator to move to the second floor where they opened the elevator doors and miner was able to safely get out at 1930.

Due to mechanical reasons, the main shaft hoist has been taken out of service at the Maysville Operation I.D. # 1507101. No miners were injured and no miners were underground when the hoist was taken out of service. After talking to employee, a call was made to report this outage.

A non-injury, unintentional roof fall has occurred on the West Mains #2 Belt just inby crosscut 17. The fall was estimated to be 30 feet long, 18 feet wide and 5 to 6 feet high. The area has been clean up and re-supported as per Recovery Plan.

The elevator would not run in inspection mode in the up direction. During the troubleshooting the elevator became inoperative at 11:40am. During the investigation it was found that the wires in the jacketed cable to the pendant controller were broken.

An unplanned Roof Fall occurred in the 3rd North Section, in the intersection of #6 Entry and crosscut #9, outby area. The area that caved is approximately 20 sq. ft. and 9 ft. high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; Primary Roof Support did not have the resin the length of the bolts as required by the Roof Control Plan.

Caught in, under or between collapsing material or buildings

Donning fall protection equipment, the miner entered the top of a primary feed hopper without the knowledge of management and contrary to mine safety procedures to remove a large rock. While trying to break up the rock, raw material that remained on the sides of the hopper sloughed off engulfing the miner.

Employee was crossing intersection and was struck by the left center shuttle car resulting in fatal injuries.

An EE was fatally injured when coal and rock material fell from the mine rib while rib bolting, and pinned the injured against the continuous mining machine. The rock and coal material measured approximately 6.3'x3.2'x2.0'. The miner was trained in the task being performed.

Employee was sliding a grating into a location. When sliding the grating, the grating slipped and pinched fingers on both hands. Lost tip of left little finger and had lacerations on the others.

Employees were repairing the latch bar on the shovel dipper. The employee was prying with a bar to loosen the piece of the bar. The dipper door moved and pinched off the fingers on the employee's left hand.

Employee was assisting in removing a conveyor belt from a chute when a chain shifted and caught employee's finger between a steel beam and the chain. Employee sought medical attention and was diagnosed with a laceration and a finger fracture and is currently losing time. *Middle finger was amputated at the pip joint on 6/22/2021.

Miner was lifting rocks into a rock bucket when they felt pain above and below right elbow. After trying to work through it for a short time, they decided they should let the foreman know and quit lifting. The foreman told them to stop working and it was decided that they should seek medical attention.

Struck by falling object

IE walked over to check the CM that was down in E2 at 14+18. As the IE approached the CM, a rock measuring (2.5-4 W x 4 L x 5-1 T) fell out from between the roof bolts. The rock struck the IE's head and right forearm pushing IE forward. To prevent from falling forward, the IE planted right leg in front of self. The rock hit the IE's right knee as it fell to the floor.

Employee was getting ready to help move the bob cat when they stepped up to get in it they heard a pop and knee hurt.

Was stacking 25lb bags. Turned to pick up bag and had pain in EE's back.

The guide chain on the forklift the employee was operating kinked at the pulley, the employee stuck EE's hand between the pulley and chain. The chain smashed the tips of the 2 middle fingers on EE's right hand.

On 6/29/21 Employee requested to be sent to the clinic for lower back pain. Employee said EE felt a tightening of lower back muscles soon after arriving home from work. Employee couldn't pinpoint any particular task/incident that could have caused the pain and they only performed normal routine duties (cleaning, sweeping shoveling, bin tender operation).

IE was working on a small access platform (2 steps) near the Courier Analyzer and turned to step off onto the walkway and misjudged the height of the step and fell onto the walkway.

employee was using a shovel to clean up on a pontoon on dredge twisted and felt sharp pain after arriving home felt more pain so went to medical facility on 6/29/2021 brought a note in reading may return to work 7/2/21.

Struck by falling object

IE was drilling shot holes when IE had to add the pusher steel. IE did not support the starter steel and it fell from the hole IE was drilling and struck the tip IE's right index finger. This caught the finger between the drill steel and the drill pot. IE was brought outside and taken to have the finger checked.

Employee was cleaning a muck spill on the crusher floor using a high pressure water hose, when the nozzle assembly separated from the hose. As EE went to shut the main water line valve off the water, the hose whipped contacting the right thumb, resulting in a fracture to the tip of EE's thumb.

IE fell on rocks landing on the right side of body. No fractures, but too sore to return to scheduled shift.

Fall from scaffolds, walkways, platforms

Employee was standing on a bar grating platform to hose off floor steel when the grating gave way causing employee's right foot to fall through a hole in the platform.

Contact with hot objects or substances

While employee was working on a water pump EE realized the system was not pumping water properly. While the employee was removing the hose connection to the water pump EE was sprayed with hot water. Employee suffered second degree burns on EE's right arm, chest and a small area on EE's back.

Employee was mucking at the Enlow 2 South #1 Airshaft Site. During the process of switching lifting hooks from one muck bucket to another employee slipped on the wet shale and rock. This caused a rolling of the right ankle beyond normal range of motion.

A piece of rock fell and struck employee on the lower left leg resulting in a fracture on the left leg/ankle.

A fully trained miner was using a track jack when the jack fell over onto EE's foot. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee was walking towards mine shop office, stopped to turn around, and foot caught on floor. From MRO report 7/14/2021, Employee fractured fibular head on left leg.

EE was walking behind the secondary crusher and had reached down to remove wheel chocks. EE fell down injuring lower lumbar, right hip and left shoulder. Medical assistance was called and the EE was transported to a hospital for treatment.

An employee was measuring silo levels, when the measuring tape got caught & employee used a long bar to move the part that was stuck. When the tape was freed, employee's right hand moved hitting the side of the silo opening causing a laceration in employee's right little finger joint. Employee received 4 sutures.

The employee had exited backhoe to help make a pipe connection and was in the process of remounting the machine. After placing right foot on the step, the employee used the grab handles and right leg to step left leg onto the track. When this happened, the employee felt something pull in right leg.

Employee was pulling themselves up as they were attempting to exit the scoop when they overexerted, causing a sprain/strain injury to left shoulder. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

While setting up a pre-fabricated metal overcast, the structure collapsed striking the injured on left ankle resulting in a fracture.

Employee lost EE's balance and fell on EE's left arm.

Miner lost balance and fell on stairs at screen tower.

Employee walking from the conveyor to the stacker stepped on uneven ground and rolled ankle resulting in a strain.

IE was changing a hydraulic hose on a shuttle car. While pulling on the hose, EE's right index finger popped a couple times and EE continued to have a burning sensation in the right wrist. IE was removed from work on 8/5/2021 by Medical until MRI is complete.

Contact with hot objects or substances

Tech was using a torch to cut out a tank on a 993K loader. Sparks and spurs came off as tech was cutting landing on pant right pant leg. Tech didn't realize pant leg was on fire until it had burnt tech. Received minor burns on finger tips as tech was putting out the fire on pant leg. Tech had taken off gloves to pat out fire.

Struck by falling object

The injured EE was setting L/W handrails with a chain hoist. As the EE was raising a hand rail into position, the handrail fell striking EE on the right thigh and foot. The injured didn't sustain any fractures, but was put off work until 6/28/21.

Miner was using a roll back truck chain hoist to lift steel when the winch cable broke, and the hook on the end of the cable flew back, striking the miner, and fracturing left cheek area.

Contact with electrical current

Employee was welding in dead cell (tied off appropriately) and welding machine was grounded appropriately. The wire to the machine was tugged on at the bottom, and the wire became compromised and shocked EE.

Employee was walking downslope at the No. 1 Drive heading to the return pump when EE slipped and strained (Left) knee. On 7/21/2021 employee began missing work because of the injury.

IE had assisted in picking up a carrier tongue to hook to a motor when IE felt a pop in lower back. IE wanted medical attention after next shift worked.

Over-exertion in pulling or pushing objects

IE was building steel sets. EE pulled on an I-Beam and felt a strain in EE's abdomen. IE was diagnosed and treated for a hernia on 9/14/2021.

Struck against stationary object

Employee turned to walk to another entry and in the process struck head against a pin board jamming neck. Employee started missing work on 6/28/21. After an investigation into the injury, it was determined to be work related on August 6, 2021.

Employee was pulling shields when a rock was thrown from the shearer drum and struck EE in the left elbow. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Caught in, under or between a moving and a stationary object

Employee was assisting bolting partner retrieve the steel out of a rib bolt hole when a piece of loose rib fell and struck EE's thumb against the drill mast. This resulted in 13 sutures.

Over-exertion in pulling or pushing objects

Miner was standing in an awkward, twisted position holding tension on a 5 foot pry bar. The bar jerked away from the miner causing them to be jerked in a twisted position. This resulted in an acute strain to the lower left back paraspinal muscle.

Going down stairs that had a thin layer of graphite on them.

Employee was laying on a creeper replacing a clutch brake. The tool slipped out of EE's hand, bounced on the ground and hit EE in the left eye.

Employee alleges was shoveling and EE heard a pop in the lower back.

EE was performing maintenance on the crusher, as EE reached for a part EE felt a strain and pain in EE's hamstring.

Engineer was climbing the steps on a locomotive when hips and back began to hurt. Later claimed to have numbness in hands and legs.

Employee did not clear frame of loader and stood up and hit above right hip. Experienced back pain and requested time off. Returned to work 6.28.21 7.8.21 Slipped on loose sand while assisting moving a drum of oil. Reaggravated injury and went to Urgent Care. On Light Duty and Worker's comp.

The injured miner was using a Caterpillar D-10T2 bulldozer to push material parallel to a bermed bench. While traveling in reverse the dozer drove off the downhill side of the bench and rolled over, ending up on its tracks. The total damage to the D-10 is unknown, but the machine appears to be structurally sound and repairable.

Employee was walking to the mantrip when EE stepped into a hole and felt a pain in right ankle. Resulting in a sprained ankle and a small avulsion fracture.

Working conditions were good. Miner was is seated stable position performing operation. Miner lost grip/control when cut-off wheel grinder kicked back below waist area and rebounded off hopper/structure striking miner along cheek resulting in laceration. Miner does not recall what caused kickback. Miner was wearing safety glasses during cutting.

While operating the Longwall shearer employee tripped and fell onto right arm. Employee returned to work next scheduled shift and worked all required shifts until being taken off for surgery to repair shoulder on 10/6/2021. Lost work days will be counted from 10/6

Employee's right foot got caught on a piece of mesh causing EE to trip. Resulted in a fracture tibia / chip fracture on base of tibia.

IE was carrying a torque shaft to the LOXC. IE went to throw the shaft down and it caught on IE's suspenders on belt. This caused the shaft to fall straight down onto the IE's right toes. IE sought medical attention on 6-21-21

Employee was tasked with replacing belts on a pump. As EE was turning the pump sheave with a pipe wrench, the momentum of sheave caused the employee's finger to get pinched between the pipe wrench handle & a belt guard mounting bracket. EE was seen by onsite EMT & sent to LCMC where x-rays showed a fracture in tip of finger. EE will have surgery on 6/23/21.

While conducting a post blast inspection the employee was scaling HW rib. EE was working a bowling ball size rock from behind the screen when a rock fell contacting the employee in the right foot.

EE was on a ladder when the legs kicked out and EE fell on EE's right arm and broke EE's wrist.

IE was loading timbers on a ride when EE's foot slipped causing EE to lose balance while holding a timber. When EE tensed up to stop self from falling, EE felt a sharp pain in the back.

The impeller shifted a little forward on the forks, and the EE got in front of the forks to reposition the impeller. IE stated that while trying to unplug the charging cable from the battery for # 14 scoop, the charging cable was stretched over the battery lids. As the IE pulled on the cable to get some slack to unplug the charging cable, the cable pulled on the lid enough to slam closed on IE's left pinky finger as EE was disconnecting the charging cable plug, causing a laceration and fracture

As the IE went to reach for the cable to move it, EE believes they hit one of the tram buttons causing the machine to shift and pin themselves between machine bumper and the coal rib. IE was treated for a fractured pelvis.

While digging on the beltline to install steel sets, a rock dislodged from the roof and struck employee on the back of the neck. Employee was wearing required PPE and injury was not the result of a training or compliance issue.

EE was using a rake to clean up the accumulations that were under the North Mains #1 Belt, the belt was approximately a foot and half off the ground. Injured and another employee were putting covers back on the Roof bolter and when they went to pick up the third cover injured said EE felt something pull in EE's left bicep area. Injured was taken for medical attention and then placed on restrictions pending MRI results.

A laborer suffered a sprain to left thumb/wrist while picking up a wheelbarrow. The miner states they felt a twinge run from thumb to elbow, they finished the shift without incident. EE reported the incident the next day as they were experiencing pain. EE did not request medical treatment until July 13, 2021 and was placed on work restrictions.

An employee received burns to both forearms when the employee attempted to put a set of scoop batteries on charge. The cathead shorted.

Employee removed seatbelt to exit the broom and got tangled in the seatbelt. Employee grabbed the side handle and injured right shoulder. Employee first sought treatment on 7-14-2021. Employee had an MRI on 7-20-2021. We received the MRI report on 7-26-2021 which showed a partial tear.

While carrying screens for switching out on dewatering screw, EE felt discomfort in groin section. EE went to hospital and was diagnosed with hernia-type injury. Surgery was performed and EE was released next day.

Employee was manually packaging 50lb. bags onto a pallet for 7 hours. Next day EE's right elbow was sore. After a few weeks, employee felt more pain. Employee reported the incident to the manager on 7/12/2021.

Employee was clearing out a plug up from the kiln feed hopper through a side hatch cover. As the employee was performing this task, hot debris came out of the hatch cover dusting the employee resulting in burns to the employee's forearms.

IE was loading a 6"" vacuum line into the back of a pick-up truck when they felt a slight ""pull"" or ""pop"" in lower back. The IE reported incident to supervisor on 6/22/2021.

Conducting secondary breaking operations. When EE was pulling the wedges the stone broke and EE fell to a lower level fracturing and dislocating EE's ankle.

Employee was pulling out of the parking lot and laid their motorcycle down in order to miss an oncoming vehicle. The employee fractured their right ankle.

EE was helping our millwright roll over a metal beam to the other side so it could be welded. When they began to roll the beam, EE's right shoulder popped.

Employee was fixing a leaking pipe near the gate valves. On the way out employee struck head going out of the tunnel into the pump house where the clearance is low. Requested medical attention on 06/29 when condition didn't improve.

Stepping down off a tail pulley guard that was 18"" off the ground. Guard was covered with rubber belting and had loose rocks on it. As EE stepped down the rock slipped and made EE slide down the side of the rock. EE caught self and this resulted in a dislocated right shoulder. It was an 18"" slip

HT216 operator was traveling empty to shovel when truck hit a soft spot in the haul road and reported lower back pain to supervisor.

Wiping dust off windows of Loader, slipped while going back into cab. Foot slipped off the step at the cab and fell to the ground.

Employee was replacing a 2' x 4' x 3/8"" piece of steel plate on a bulldozer blade. The employee stood the plate up resting the 2' edge on the floor and begin walking the plate corner to corner to the center of the blade. Once they got to the center of the blade they began laying the plate down on its opposite edge (4' side)when the injured employee felt a pull in left arm.

Absorption of radiations, caustics, toxic and noxious substances

Employee was stung by a bee while traveling in an open cab utility vehicle. Employee is allergic and administered personal epi pen. Employee follow up with doctor and was required to be off work.

Operator began to maneuver drill from the low-boy. The spotter signaled operator to stop, operator engaged the parking brake, unbuckled seat belt, stood up to access the situation. At this point the cribbing rolled, shifting the front tire off the trailer. At this same time the operator exited the cab and sustained an injury to heel.

While mucking the 1400 mpc round out the bucket of UL20 mucker caught the rib and stopped the mucker instantly jarring the operators neck and lower back. The employee went to the ER the day they reported it they wanted EE to be released by an orthoepic, the worker comp case manager could not get EE an appointment until 7-21-21 with an orthoepic.

Employee was operating the overhead crane via a remote control box with their right hand and maneuvering a discharge liner with their left hand. Tip of left hand ring finger was pinched between the liner being lifted and a stationary liner positioned in the storage rack.

On 6/9/21 an EE alleged while walking across the catwalk and stepping down on the first step the employee twisted right knee. The EE didn't go the doctor but, spoke with on the site nurse. The EE continued working but, on the 14th, wanted to be seen by a doctor. The onsite nurse scheduled an appointment for the employee on June 15th, restrictions were given then.

Injured was walking the M2 belt line and EE's knee locked up and EE fell to the ground and when EE fell EE's left knee hit a piece of C-Channel. The employees left knee was x-rayed and it was discovered EE had broken EE's knee cap.

Injured worker was using a hand held shovel and as they stepped on it to dig they strained their left calf muscle.

The employee was working in the mill room and when they turned they tripped over a cord falling on left side causing discomfort to hip left hip and thigh.

During a crusher steel change a miner was removing a plate from a mandrel. In doing so they pulled the snap ring that holds the plate onto the mandrel. The plate released and struck the employee in the hand causing a contusion.

Employee was washing slurry from floor under moly cleaners during regular shift when they slipped and fell hurting their back.

Employee was hooking up screening plant to loader hitch. While placing pin in hitch, loader shifted forward, causing pin to tilt in hitch, resulting in employees fingers being pinched between hitch pin and counterweight of loader.

While hand palletizing Flat rock / landscape stone EE felt like EE pulled a muscle in EE's left groin area. EE continued to work the rest of that day. That was on a Friday. Monday EE came back to work and worked a 1/2 day and alerted us to the accident EE had on Friday. I sent EE to the ER to get checked out for a possible hernia

Ee was lowering a turnstile head when it slipped and slid down EE's arm, causing a laceration. Seen at NNRH and referred to Renown ER. Renown ER referred for ortho consult. Ee released to restricted duty which site could accommodate effective 6/4/21.

2 employees working, one running skid steer, pulling axle from beneath crusher. Injured person reached beneath axle, and other employee accidentally moved skid steer forward and pinned injured person's hand. Bone between wrist and knuckle on little finder was broken.

EE was assisting forklift to turnover cheekplate. The forklift operator got the ckeekplate picked up on one side to about 85 degrees. The EE began to push on the cheekplate. The forks lost tension under the plate, popped up, knocked the EE's hand loose from the plate, and the plate fell back on the EE's left, lower leg causing a fracture of the two bones.

EE was bent over to pick up a stone to place in the basket of the skid steer when EE felt a pain in the belly button area.

Employee Stated they had adjusted the drill pot on a DDr13 roof bolter and was watching it while bolt partner raised the drill pot and got the tip of left pinky finger caught between the longarm and the drill pot. Employee got medical treatment on 7/12/21 and was placed on restricted duty.

While trying to reposition haul truck drive shaft, the shaft slipped, pinching the finger between shaft and yoke on the transmission.

EE had hooked EE's hoses up to unload. EE went to wash EE's boots, turned to check EE's load, caught EE's right foot on EE's wheel chock and lost EE's balance. EE fell on the concrete, landing with all EE's weight on EE's right knee. EE couldn't walk on it.

The employee was cutting 2"" x 2"" x 1/4"" square tubing for handrail supports and catwalk structure. The employee had cut three pieces 3'6"" long. Each stick weighed approximately 19 pounds. The employee grabbed three sticks of the tubing and carried them up the stairs to the catwalk. As EE was walking up the steps EE felt a pain in EE's lower back.

EE was placing a fan blade on the maintenance shop table when it slipped and dropped causing a laceration to EE's index finger on EE's left hand. After removing the fan blade from the motor shaft, EE removed EE's gloves to take a break but then decided to sit the fan blade on the table to get it off the floor.

Employee stated EE's back hurt from climbing in and out of equipment. Employee received medical treatment on 7/14/2021 was released to full duty and again on 7/27/2021 when EE was given work restrictions.

Employee was using a bar to remove a rock that had become jammed in the Kodiak K400 Cone Crusher. The employee hit the rock with the bar; the bar came back, striking the employee in the face, resulting in a split upper lip on the left side of EE's face. While removing a drilling hose from the flood shaft hole, the hose pressurized due to being capped off. When the cap was removed and pressure relieved, the cap flew off and struck a nearby employee in the ankle. The employee had a small abrasion wound in the ankle and ankle soreness the following day.

While cleaning CIL containment the employee attempted to lift a wash down hose that was buried in the mud and strained EE's lower back. A full duty release was received at the initial Dr. visit. On July 1, 2021 EE attended a follow up appt receiving a light duty release hence making this injury a medical reportable injury.

EE was bolting top and was on EE's second row of bolts, The EE was in the process of spot bolting the corner, when the corner broke out striking EE on EE's left side.

Employee was using a skid steer to clean up core sample bags. While exiting the equipment to load bags into the bucket, EE stepped on the edge of one of the sample bags and twisted ankle. The employee was placed on modified work duties.

While riding in a man lift, a 3 foot piece of I-beam fell over and cut lower leg of Employee.

The injured was moving rocks off a water hose at the primary crusher when the rocks shifted pinching right little finger between two rocks resulting in the wound. The injured was wearing gloves at the time of the incident.

Employee was repairing the grates in the Kiln 3 Cooler. As the employee was in the under grate section, clinker material fell down/breaking (hot center) into the cooler onto our employee causing a burn to the back of the neck.

The employee was walking up a set of open ended steps when EE's left boot got caught on the step causing EE to fall forward. When EE fell forward, EE put all of EE's weight on the left shin. A large contusion formed on EE's left shin.

A haul truck operator hit a rough spot at the American Mountain pit intersection, and the truck rocked causing back and shoulder pain.

Miner attempted to step on a short ladder leading to a conveyor service platform. Left foot slipped on ladder rung and right shin and knee hit the service platform mezzanine causing trauma to the knee cap. Miner did not have left foot in full contact with the ladder rung, only the tip of EE's boot. Ladder missing hand extension on the West side so did not have 3pt. contact.

In the process of moving a length of 4 inch HDPE pipe, the employee lifted the pipe over EE's head and walked forward, employee lowered the pipe to the ground. An hour later the employee felt a pop and pain in the stomach and groin area.

An employee was climbing a short set of stairs into the office building when EE missed a step causing EE to fall forward. EE caught self on left hand causing a wrist sprain.

The incident happened on 6/14/21 and turned into a reportable incident on 6/17/21. The employee was using a torch and cutting above shoulder height above EE when a spark entered EE's right ear.

EE was holding onto a shackle while getting down from a roll assembly. The tip of EE's left middle finger got stuck in the pin hole of the shackle (the pin was removed) and EE fractured the tip of the left middle finger. EE was diagnosed with a fracture of the left middle finger tip and returned to work with restrictions.

The employee was cleaning a pedestal floor plate w/a grinder and wire wheel when foreign material got into EE's right eye. The employee was wearing Safety Glasses and a Face Shield at the time of the incident. EE immediately came down from the machine and flushed EE's eye to remove the material from EE's eye. I was notified and continued flushing the eye with eyewash.

The employee was assisting on getting the grinding aid pumps flowing and was turning around to walk back to the control room when EE tripped over a short stool that was left in the walkway falling and catching EE's right leg between the stool and the frame to the grinding aid pumps.

One employee squatted down to pick up the backstop, which was in a 12""x12"" box and weighed close to 100lbs. Upon squatting down the employee had to adjust position due to being uncomfortable in the coveralls. Once the employee adjusted position, EE went to lift the box containing the backstop and felt a small tug in lower back.

While bolting the #4 entry on South Mains Section, the employee was drilling the 2nd rib bolt and as EE was letting the drill pot down EE slipped and as EE tried to catch self, left hand came in contact with the drill mast causing a laceration to the tip of left middle finger which took 9 sutures to close.

EE was adjusting the Gamma machine and requested that teammate start the machine and EE's right hand was still in the machine. EE's right hand became entangled between the two railings.

Came in next night and worked until about 11 pm, and told supervisor they wanted to get the knee looked at because it was still swollen. EE went clinic to be seen.

Miner stacked pallets 20 high for use in packaging supersacks. When they reached for the top one a foot got caught under the pallet stack and when they went to step back was unable to. They fell back on to the roller conveyor and sustained a back contusion and sprain of ligaments of thoracic spine.

On Friday June 4th, an employee felt a pop in the elbow when EE picked up a bucket of crusher backing material. After still experiencing pain on June 24, EE decided to seek medical attention, and was given work restrictions due to inflammation in EE's elbow.

While handing the welders the third grizzly bar the employee stated that when EE lifted it, EE felt a slight pain in the left knee. After doing so, the employee noted irritation and possible small debris in both eyes. The repairman grabbed a bottle of water and began flushing their eyes.

While working on Drill 14, the employee was working out of a man basket attempting to remove a pin from a cylinder. The hammer got caught on the safety cable for the JLG controls causing the magnetic base to become loose and strike the employee in the left eye. This caused a small laceration.

Hot sunny day. Working under the new fan house blowing holes in a metal plate for a power center. Employee was using torches while wearing all PPE. When completed task, employee was walking out of fan housing. Hard hat feel of when employee went to duck under a 62 1/2 beam. employee struck head on beam resulting in a 2 1/2 laceration on top of head.

Struck by falling object

While employee was unloading a bucket of hydraulic fluid from the back of truck, the plastic rim of the bucket broke causing the metal handle that the employee was holding to slip. While in the process of slipping the metal handle (hook on handle) caused laceration to employee's right index finger resulting in stitches.

Struck against stationary object

Employee reports EE was checking a hopper and stepped on a piece of aggregate material while taking a step back. Employee reports hearing a pop and twisted EE's left knee. Employee was put on restrictions and referred to an orthopedist after being diagnosed with a left knee sprain.

Welder struck in face with line boring hydraulic drive motor couplings.

Operator was descending from a railcar after setting handbrake, but did not step properly causing them to roll left ankle. NOTE: Employee failed to return to work with modified work available, stopped communicating with company - job abandonment case and termination- ""Return to Duty"" info has been adjusted so no ""lost"" days associated with injury reported.

Caught in, under or between a moving and a stationary object

Employee pinched finger while helping change out a belt roller. Received 5 stitches, and x-rays revealed a small fracture.

The employee was setting a new roll of shrink wrap onto the bracket. The employee did not move hand out of the way when EE laid the metal pipe (that goes through the middle of the roll) down onto the bracket. The employee's middle finger got caught in this pinch point which caused a laceration to the middle finger. The employee received several stitches to close the wound.

Struck by falling object

Installing a 2000 amp breaker. When placing the breaker into its location it fell on EE's right index finger. Causing a laceration and getting five stitches.

Struck by flying object

A piece of metal flew from rollers and went through EE's coveralls and embedded itself in EE's leg.

Employee injured left thumb while changing belts on #46 Blower. Employee's Left Thumb was caught between the Chiv and Belt, system was de-energized.

Fall to the walkway or working surface

A contractor was walking across the parking lot before the start of shift. They tripped and fell. This action resulted in a cut to the right hand that required sutures.

Struck by falling object

Employee was putting new gears in a shaker. The gears slipped off of the shaft and employee's right thumb got caught between the gears and the shaft when they fell. Employee's right thumb was fractured and cut.

Employee was cleaning up around wash plant and a rock fell off the plant striking employee's forehead. Employee received a small laceration.

Cutting conveyor belt with a razor knife and blade slipped and cut EEs leg just above the knee.
