A unplanned roof fall occurred in the #1 entry 1 x-cut outby. The 001 section at 0500, the size was 18'Wx20'Lx14'H. At spad #3292 in the return. The state and MSHA were both notified at 0515. The fall was lceaned up and bolted.

Hoist kicked due to a ""Drive Converter A"" fault. The hoist was reset but had to be re-synced before it was put back into service. Hoist was back in service at 1:35pm. There were still 2 operable hoists.
An unintentional roof fall occurred on the 2A travelway entry #5 and #6 at station 5+90. The fall did not affect ventilation or cause personal injury the fall did affect the secondary escape route which was re-routed. The fall measured 85 feet x 18 feet x 4 to 6 feet high.
Hoist kicked due to ""West brake"" fault. Mechanic was called out of the mine to check the brakes. Electrician reset the hoist and was back in service at 4:45 pm. There were still 2 operable hoists.
#3 unit neutrals, x-cut 8, #6 entry. 30' Length, 19' width and 8' thick.
A roof fall was found by a MSHA inspector and the Mine examiner, while traveling the return air way. The roof fall is an extension of a previous fall that was reported at an earlier date. The location is adjacent to 5B belt near spad #22048.
Rock fall occured in the 1st northeast at spad 50+40. roof is off, shale type. fall size is approx. 20'x60'x6' and approx 6000' from active working faces. Fall occurred in out #5 entry of the subman.
A previous fall had been supported with a tunnel liner. More rock fell from the previous fall cavity and caused the existing liners to bend, blocking the secondary escapeway. Area was last traveled at or about 5:00 AM.
#3 hoist tripped due to a spear point monitor fault. An electrician & maintenance co-ordinator had to come out of the mine to reset the hoist. Hoist was back in service at 4:45 a.m. There were still two operable hoists at the time of the incident.
An unintentional roof fall occurred in the #11 entry 30 feet outby spad #3690. The fall measured approximately 25'-30' long x 17' wide x 4'-6' high. The affected area was dangered off.
A roof fall occurred on the #3 intersection on the #3 section 1 x-cut outby spad #46076. The fall measured to be about 20' wide, 30' long and 8' thick. The area was bolted with 5' trouge tension bolts. The fall will be cleaned up and re-bolted and cribbed.
An unintentional fall occurred in the # 3 entry of the 2nd main west entry's. The fall was 19' X 40' X 11'. This area had 6' tension rebar bolts and 10' cable bolts.
Roof Fall 18' x 30' x 5' in the face of # 8 entry # 9 crosscut (LOB) The fall was above the anchorage of the permanent supports.
A fall of roof has occurred above the anchorage point in 1st East # 3 entry , cross cut 15. The fall measured 45 feet in length x 19 feet in width, and the hieght is unknown. The area was supported with 6 foot fully grouted bolts.
Unplanned roof fall was discovered at in the intersection of the return airway at spad # 3018-# 3019. The area will not be rehabilited and does not effect the airway. It has been breakered off. The measurements are unknown.
While mining #6 lift to rt. in #2 hg 5-6 bl. S. Main D-F panel. Miner was backing out of left and top fell in, covering up about 10' of the cont. miner. Area posted, retrieved C.M. Cover 250'-area bolted with 6' resin bolts-called MSHA Hotline.
Storm caused an electrical circuit to blow. The circuit was replaced. Accident was phoned to MSHA. Hoist was down until 5:00 pm.
A roof fall occured on the #3 unit in the 3rd NW sub panel one break outby last open x-cut in the #1 entry. Ventilation, Personnel or equipment was not affected.
At approx. 1:08 a roof fall was discovered at the 2nd panel on the left of Rightfork Submains. The fall was 5' high x 20' wide 50' long. Ventilation was not affected; no equipment involved. Top was laminated. Additional roof support of cribs and beams has been placed in affected area.
Unplanned roof fall above anchorage zone in the intersection of the 4th N Butt and 4th NE Butt #2 Entries. Fall is located at survey spad #3553 and #3556. The roof fall is approx 80' long X 19' wide to 30' wide at intersection and approx 5' high. Roof was bolted with 4' Install II bolts.
An unplanned roof fall occurred in the #2 gas mine in the old 1st right section, near spad #305 in the #3 entry that measured 23' long X 22' wide X 10' high. No injuries were reported. Area was developed 3/15/08. Fall did not block passage of people or ventilation.
ROCK FALL; in main return on belt 2A, 20' X 70' X 5' high. No equipment involved, no ventilation affected. MSHA & OMSL notified, will rehab the area; clean up and install additional support.
During weekly performance testing and routine maintenance, one of the brakes overheated and locked on the Timberland Hoist situated at #10 Shaft. All miners working underground were safely evacuated to the surface via #8 Shaft. After the brake was allowed to cool, it was adjusted, tested, and the hoist returned to service.
ROOF FALL; Intake at spad #19536, 25' X 20' X 5 1/2 > 7' high. Area affected no ventialation or equipment, was breakered off, will not be rehabed. MSHA & OMSL were notified.
A non-injury roof fall occurred during rehabilitation work of the North mains. 18 steel arches had been installed between 6 and 5 x-cut in the #5 entry. A crib had been removed to allow more steel arches to be installed in the #5 xcut intersection. scoop brought to crib and arch, top started working 20 min later top fell. 6-8ft high, 35' long anf 18' wide.
The Livingston Portal elevator ceased operation at 7:45 AM on it's way up the shaft from the shaft bottom. There were 10 men that were riding the elevator at the time. The elevator was able to be brought to the surface at 8:45 AM and the men were safely removed. There were no injuries as a result of this incident.
A roof fall occurred on a roof bolting machine in a fully supported crosscut. The fall was 80' long, 18.5' wide and 5' high. The area had been bolted with 5/8"" x 4' fully grouted resin bolts. The fall was in crosscut 3-2 on the 0010-0 section.
Production hoist tripped out in automatic due to tachometer fault. MSHA notified by phone at 6:48 AM and mine evacuated immediatley. Employees returned to work at 7:30 AM after tachometer was replaced and test runs were complete.
Following our previous evacuation the production hoist kicked out with same fault as indicated earlier. MSHA was notified immediately and mine was evacuated. Tachometer was replaced again and wiring was repaired. Employees returned to the mine after test runs were completed.
An unplanned roof fall occurred in the #130 mine on #5, brk 26, #6 entry that measured 40' long x 20' wide x 8' high. No injuries were reported. The area was developed on 7/10/06.
A roof fall was discovered in the 2 North 1 Mains 11 crosscut #1 entry. The fall is approximately 26' long, 18' wide & 6' thick. The area is supported with 5' fully grouted resin bolts.
A roof fall was found on 7/28/08 at 6:20 am by an examiner inspecting a set of seals. The fall which measured 8' high x 20' wide, and 40' long was in a four way intersection at 40+60 entry 0 in 5rms. Area had been mined 3/2003 and was supported with 8' full resin bolts. no injury resulted and fall will not be cleaned up. Fall was reported because it was adjacent to examiner
NO INJURY, employee was tramming 330DL Track hoe when he struck an overhead powerline. Powerline was severed. NO ONE INJURED. Lines being damaged caused units to trip inside of Plant.
Excessive moisture accumulation in a switch, causing it to fail.
EE drove the ride 2000' with a flat tire and melted the battery down and caught the ride on fire.
The immediate roof broke, sagged, and set down on the stageloader conveyor assembly and impeded travel between the head gate shields and the filter station for distance of 25"" to 30'.
A roof fall occurred on the #3 unit outby the tail piece in the #5 entry in the 3rd nw sub panel. Ventilation, personnel, nor equipment was involved. Length 20' x width 19' x thickness 6'.
A roof fall occurred on the #3 unit in the 3rd nw sub panel in the #6 entry outby the tail piece. Length of fall was 90' x width 19.5' x thickness 5'. There was no equipment involved in the fall. Ventilation was not effected nor was personnel.
VR-4 SECONDARY EXHAUST FAN WENT DOWN AT 3:10 AM DUE TO OVER VOLTAGE. ALL EQUIPMENT WAS SHUT OFF AND CREWS REMOVED TO SURFACE. THE FAN WAS BACK RUNNING AT 5:30 AM AND NORMAL OPERATIONS RESUMED.
At 6:32AM, the fireboss walking the bleeders reported a fall at #33 break. 20'Wx100'Lx5'T. area is 8990' from the active face. Travel way nor ventilation was not affected.
An unintentional roof fall occurred on the 1st North Panel (#1 Unit 001) off the Main East in the #6 entry at 8+40 cross cut. The fall was approximately 25' wide by 8' high. MSHA was notified within the prescribed time frame, but I failed to file a 7000-1 form. This is being sent to abate a citation.
While doing and E01 inspection of the bleeder system on the 11a southeast panel, a roof fall was found. The fall was located 3 breaks inby survey station # 2535 in the #8 entry. The fall was in the intersection and measured approx. 30' long x 19' wide x 4' high.
At 11:15 pm the Silver Shaft Hoistman started to get emergency stops on the north skip around the 5900 level while hoisting rock. It appeared that the problem was electrical. At 11:45 the mine was completely evacuated via the #2 shaft. At 5:45 am the electrical problem was diagnosed and the Silver Shaft resumed operation. A mechanical relay switch was in the wrong position.
A roof fall occurred 1 break inby spad #107 in the return airway. The fall measured 15' wide, 40' long and 4' thick. Persons, equipment nor vetilation were affected.
An unintentional roof fall occurred approx at 5:00am on 7/25/08. The location of fall adjacent to spad 3104 in #2 AC in intersection. Area dangered off, then cribbed off and rest of surrounding area's re-evaluated and extra supports added.
At approximately 4:50am the loading gate for the N skip became stuck, halting operation of the hoist. The hoist operator failed to follow established procedure to notify management, who learned of the problem at approximately 6:05-6:10am. Action was taken and the hoist became operable at 6:20am. No injuries or other problems.
While doing an E01 inspection of the bleeder system on the 11a southwest panel, a roof fall was found. The fall was located 3 breaks inby survey station # 24977 beginning in the X-cut between #5 & #6 and ending between #7 & #8. The fall was approx. 140' long x 19' wide x 8' high.
At around 10:45 am the Hart Street elevator malfunctioned. A brake shoe got out of place on the cage causing the malfunction. The elevator failed to stop at the proper level at the bottom landing and had to be raised manually.
At approximately 5:10 AM on 07/24/2008 AM the kiln operator noticed a spike in the temperatures in the coal mill. A piece of metal had entered with the coal causing a fire. The operator shut off the air supply, called the fire department and notified MSHA. The fire was contained in the coal mill and was extinguished soon after.
A roof fall occurred 1 break inby spad 1225 in #3 entry. It was approximately 60' long, 18' wide and 1'-12' thick.
The slope hoist car was descending down the slope and around 531 feet a DC fault occurred and the computer shut down the hoist. After trouble shooting for the cause since the fault would not re-set a wire was found to have come loose on the repeater. The slope hoist was down for more than 30 minutes due to this problem, there was no accident that occurred.
The superintendent was walking the returnes and discovered a roof fall. The fall measured approx. 6' in height and included an area about 18 ft wide by 18ft long. The fall in the returns off of the main section in the #2 entry. located at/near spad 1242.
A roof fall occurred outby the No. 4 unit, 2nd south panel, in the No. 2 return entry. The fall extended from spad No. 5+55 to 7+60 and measured approximately 245' X 18' X 6-8' high. The fall was located approximately 1000' outby the no. 4 unit dumping point.

An unintentional fall occurred in the 1st N. Submains. The fall was 80' X 18' X 8'. The fall fell in the 24+20' intersection and went outby for 80'.
A roof fall occurred on south E #6 hdg between 16 & 17 x. 20' long 7' high, rib to rib. Area was bolted with 5' fully grouted roof bolts and 12' passive cable bolts. No men or machines were injured or trapped. No ventilation interupted. Had to re-route lifeline to #7 hdg.
An unintentional roof fall occurred at the Galatia Mine New Era Portal at the tailgate of the 2W longwall. The fall was approximately 25' long, 5' - 6' wide & 8' thick. The fall did not affect ventilation. The fall will not be cleaned up.
South 4 - between 26 & 27 block, #3 entry. Support: 6 ft 3/4 inch resin bolts, pans, brow straps, jacks. 30 ft long x 17 ft wide x 8 ft high. Fall dangered off, additional roof support installed around fall. Fall cleanup plan posted.
A Oil line broke down under the cab of a D11R dozer. The oil was ignited by short in a wire in the wiring harness. The fire was put out twice but kept igniting. No was hurt but the Dozer burnt up.
Our examiners found a roof fall in the 4L4MN panel. The fall was located in #4 entry at crosscut 38. The fall was located in the next crosscut inby from the examiners route of travel.
A roof fall occurred next to spad 2450 between #3 entry and #2 entry. It was approximately 20 feet long, 19 feet wide & 3 feet to 6 feet thick.

An unplaned roof fall occurred in the #4 entry of Spad # 1603. From longwall face to 10 feet outby. Fall measured 10 foot long, 20 foot wide. Ventilation was not affected. No persons or equipment were involved. The fall will not be rehabilitated but should mine through fall in about 3 passes.
We had a rock fall in the old abandoned 002 section in a crosscut between the no. 2 & 3 entries between spad no. 6072 & 6073. It does not block any airways or travelways. This fall is from 2' to 7' high & about 18' wide, 35' long. The fall may have been caused by an undetected faulty area along with a block of coal left in the next coal seam. It will not be cleaned up.
An unintentional roof fall occurred on the old 1A return at station 37+80 entry #9. The fall did not affect ventilation, passage of men or cause personal injury and it will not be cleaned up. It measured 20' x 18' x 4'.
An unplanned roof fall was discovered in the return area of an old panel of rooms.
This is for an unintentional on 1st left panel off of #5 belt #3 entry spad 3018. Does not affect travel or air. Approximately 60-70 feet long, 18 feet wide. Above anchor. Fall will not be cleaned up. Fall is timbered and dangered off.
This area had experienced a previous roof fall and area was re-supported with a steel arch support. A piece of rock has fallen and struck the arch structure. The arch prevented loose rock from damaging the beltline. A 8' x 12' section of the arch was damaged. 2 west beltline was not damaged.
A roof fall was discovered on 7/22/2008 at 4:05 pm in #6 entry on S W Mains outby S.S. 4600 aprox. 3,000 feet outby the working sections. The fall was approx. 18 ft wide, 10 ft long and 0' to 7' feet thick. No equiptment, persons, nor ventilation was effected. This area was supported with 6 foot resin bolts fully grouted and 12 foot cable bolts. The fall will not be cleaned up.
Roof fall measuring 28 ft X 20 ft X 6 ft occurred in #3 entry intake at spad #19535. Area was not breakered off, and will not be rehabilitated. We will crib and danger off the entries to the fall, clean, spot bolt, and re-route the intake. Both MSHA and state agencies were notified.

A mine manager and MSHA inspector found a roof fall in the 2L1R/3L4MN panel. The roof fall was located in an area adjacent to the examiners route of travel. The cause of the fall was weak laminated shale, associated with a geologic structure.
A fall occured in belt entry without any warning. Approx 60' long and 12' high. Inby Spad 4904.
A roof fall occurred at 1:45 AM in the 5 West off Cool Hill 3 Longwall Section. Fall occurred outby the section a 20 1/2 xcut (42+84 ft) in the #3 Intake Entry. Fall was 20' long, 17' wide, and 8' high. Fall blocked the passage of men in the Intake. Area was bolted with 8 feet Instal III Bolts. Area was mined approximately 03/20/08. No injuries occurred.
Intermittent fault in master relay caused irregular operating conditions into next day. Second egress from mine was never compromised,'Chippy' and #3 remained available & in service.(Hoist taken out of regular operation as mine-wide maintenance shutdown takes effect at end of 1st shift August 1, 2008. Opportunities to perform maintenance and upgrades.)

A rock fall occurred in the #1 entry 7+92 7th subpanel north. The fall was in an intersection and measured 10 x 19.1 x 20. This area was bolted with 6' resin bolts.
Roof fall was found @ 9:10 AM on 7/21/2008. Fall was 4.5' High X 20' X 20'. It was located near spad #69.
Roof fall occurred at No. 8 stopping outby spad 16+81 in the 9 I tailgate. Fall in the #3 entry was 16 ft wide 30 ft long and 10 to 12 ft high; fall extended into the crosscut 3 to 2 for about 40 ft.
Employee was descending down the elevator when a electrical storm was passsing through the area. The electrical storm knocked out the mine and building power and the elevator stopped. Power did not come back on until 5:20pm at that time the person was removed from the elevator. No damage occurred to the elevator and was checked by an MSHA inspector.
Rock fall occurred between crosscut #2 and crosscut #4 on the #14A conveyor belt. The fall measured approximately 120' long x 20' wide x 2'-10' thick.
On Sunday July 20, 2008 severe weather conditions caused a loss of power at the Powhaton No.6 Mine. One employee was riding the #2 portal elevator from underground to the surface when power was lost. That employee was stuck on the elevator fro approx one hour and forty minutes. As soon as power was restored, the employee exited the elevator. No injury/illness.
The primary hoist was down from 7:15 p.m. due to a lead being burned off in the field motor leads area. Troubleshooting located the problem and the hoist was running again at 11:00 p.m.
An unintentional roof fall occurred at the Galatia Mine New Era Portal at the 3W Headgate #1 Entry at 5850'. The fall was approximately 35' long, 15 - 18' wide & 12' thick. The fall did not affect ventilation or passage of people. The fall will be cleaned up.
An gate between the lime kiln and the stone feed weigh belt was not closed and the stone feed weigh belt was exposed to hot kiln gas and caught on fire. Attempts were made to extinguish the fire but due to winds, the fire continued to smolder and required more lenghtly fire suppression as personnel could not disturb embers behind the conveyor belt guarding until cooled.
Our exmainers found a roof fall during the weekly examination of the 5 West panel. The fall was located in entry #2 at crosscut 45. This fall was located in the entry adjacent to the examiners route of travel.
Roof fall measuring 20 ft X 19 ft X 5 ft 7 in occurred outby #1 entry at spad #17305. There was no equip involved, in the return airway, ventiltion was not affected, passage of persons was affected. Area was breakered off, and will be rehabilitated. Both MSHA and state agency was notified.
An unintentional roof fall occurred in the 1st North Panel (#1 Unit 001) off the Main East in the #4 entry at the #24 cross cut +1440'. The fall was approximately 24' wide by 10' high. MSHA was notified within the prescribe time frame, but I failed to file a 7000-1 form. This is being submitted to abate a citation.
A roof fall occurred on the No. 1 section in the number 3 entry cross cut at spad 7135. The roof fall measured approximately 30 feet long, 30 feet wide, and approximately 9 feet in height. The roof fall fell on the No.2 MBC, No. 3 MBC,and portion of the belt tailpiece. The No.2 MBC operator was trapped inside the operator compartment for a brief time span but suffered no injuries.
The fireboss discovered #43 intersection, #2 entry of 8N Bleeders had fallen approximately 30'lx20'wx15'h while making his weely walk. Quanity and quality of air was not affected. The area has been breakered off and routed over into #1 entry and around the fall. It is 5600' from 9E cm section.
A fall of roof occurred on #6 unit in the x-cut between the #4 and #5 entry in line with the feeder. Length 50' x width 20' x thickness 5' to 8'. Ventilation, personnel, equipment was not affected.
Two employees were entrapped in the west elevator at the mill for approximately 2 hours. Neither sustained injury. This is a follow up to the phone call reporting the incident.
An unintentional roof fall was discovered on 4 north mains at crosscut # 93 in the #2 entry survey station # 8419. The fall measured approx 20' x 20' x 8' high. The fall has been dangered off and additional supports implemented on all sides. No one was injured, ventilation was not interupted and no equipment was caught. This fall will not be cleaned up.
Our examiners found a roof fall during their weekly examination of a worked out area. The fall was located in the 1 Right, 3 Left, 5 West panel at crosscut 9 between entries 5 and 6. The fall was located in a crosscut adjacent to the examiners route of travel.

Employee was cutting on a pump shaft on a work bench with an oxy/acet torch when he heard a pop. He attempted to extinguish the fire but could not. He evacuated the building and called the fire department. The fire department recommended the gas bottles burn off. Possibly a leak in the hose or slag created a leak.
Had a roof fall above anchorage in No. 7 Entry one break inby survey station 258. There was a slick sided slip that turned loose. 18' wide - 50' long - 7' high. This fall was not in face and did not hinder travel to and from face area.
A roof fall occurred in an abandoned area of 1 Left Panel at #20 Break in the #1 Entry.
Roof fall 1 break inside. The fall was 3' high, 8' width and 18 feet in length. The fall is dangered off and supported as per roof control plan. The mine is non-producing Intermittent. Work: pumping water.
A fall of roof occurred at the intersection, in # 6 entry, cross cut 31 , station 20+20 in the South East Submain. The fall was approximately 25 feet in length x 25 feet in width and the height is unknown. The area was supported with 6 foot fully grouted roof bolts.
There is a roof fall in the 2nd Northwest Parallel, #5 unit, #3 entry, crosscut 39 at spad 27+30. The fall is 20' wide, 20' long and 8' high.
A fall of roof occurred #6 unit #3 entry 1 x-cut behind tailpiece blocking escapeway. Escapeway was re-routed. Cribbed and timbered around. Length 120' width 19.5' thickness 5'. There were no personnel affected by the fall, ventilation was not affected nor was there any equipment affected.

6 Mains 2, 9 block, 3 entry, MMU 007. Support: 6' 3/4"" resin bolts, 12' cable bolts, pans and brow straps. 20' long X 20' wide X 8' high. Fall dangered off, additional roof support installed around fall.
Had a continutation of fall reported on 7-12-08 extending thru intersection atBrk #8 #4 Entry across to #5 Entry. Approx. 40' by 20' by 5'. Area was examined by both Federal roof control inspectors and WV State Inspector.
A fire occurred on a road grader. The mine was evacuated with no injury to any person. The fire was allowed to extinguish itself. Air quality checks were made throughout the mine prior to miners being allowed to return underground.
The # 2 entry cross cut fell in at spad 200. We set built cribs and set timbers.
A fall was discovered in a crosscut adjacent to the No. 1 entry of No. 3 west marker about 90 feet inby survey station No. 5387. The fall was about 2,000 feet outby the MMU 003-0 and measured approximately 15' long x 8' wide x 6' high. The fall will not be cleaned up. There were no injuries or equipment involved.
A roof fall was discovered in #9 entry 17' inby station #103 in the return of the 001-0 section. The fall was 50' long, 16' wide and 6' high.
The crane operator was booming the crane boom up, when something ""broke loose"" allowing the boom to begin falling downword. The operator tried to catch the falling boom with the friction braking system. As the boom fell it struck a concrete peir causing the lower chord to buckle and the boom fall to the ground.
A roof fall was discovered in #8 entry 15' inby sta #80 in the return on the 001-0 section. The fall was 16' wide, 70' long and 6' high. The area had been bolted with 5' resin bolts.
Roof fall in the # 6 entry at the last open cross cut. The fall was 80' in by spad # 2638. Fall was 20' long, 20' wide and 7' high. No injuries occurred, no equiptment was involved and passage was not blocked.
An unintentional fall occurred in the 2nd west entry's. The fall fell from #7 entry, 11+20' intersection over to #8 entry 11+20'. The fall was 70' long by 19' wide by approx. 8' high.

A roof fall was found in #4 butt of 7 rooms 50' inby station 4742. 160'Long x 19.5 wide x undetermined height. The area was bolted with 6' fully grouted torque tension bolts. The area is dangered off and does not impede ventilation or travel.
Roof fall in the last open cross cut of # 4 entry, 90' out by spad 2634. Fall was 52' long, 18' wide and 8' to 9' high. No injuries occurred and passage was not blocked
Roof fall #3 entry between break 17 and 18 of the 2-Left Submain, initial fall 20' wide, 40' long and 5' in height above bolted anchorage(42"") No one injured, fell on conveyor belt and belt structure. Did not affect ventilation or escapeways.
Roof fall occured on 7/12/08, no travel way or air way was blocked. No injuries occurred from the fall, the fall occurred on 3rt off of 5rt mains on spad 2713 in the intersection. The fall measured 20ft wide by 20ft in lengh and fell from above anchor point.
A roof fall occured in the South Sub Main, entry #3 at x-cut 40. The fall measured 20' wide x 20' long x 8' high. It did not affect ventilation travel or cause injury. The fall will not be cleaned up.

A roof fall occurred at #92 Cross-cut along 33 Mains track haulage. This was a complete failure of the intersection. There were no persons injured or equipment damaged in this accident.
AT APPROXIMATELY 2:00 PM A ROCK FALL WAS DISCOVERED IN THE NUMBER FOUR (4) ENTRY APPROXIMATELY 70' LONG 20' WIDE AND ABOUT 4.5 FEET THICK. THE ROOF WAS SHALE AND APPEARED TO BE A HORSE BACK WITH SLICK SIDES.
The hoist operator of the Jewell Shaft double drum hoist was lowering three employees down the Jewell Shaft when he noticed a spike in the reading on the hoist amp guage. He immeditely stopped the hoist and the hoist was not moved again until personnel went down on the chippy hoist to do a visual inspection of the shaft & the skip the employee's were riding on.
A roof fall occurred on the main track to 003-0 section. The estimated dimensions were 6' x 20' x 30'. No injuries were reported as a result of the roof fall. This fall is in the process of being cleaned.

At approximatley 1:00 PM black smoke was observed coming off of the North West corner of the Soda Ash Production building. Surface alarm systems were activated and personnel were evacuated. Emergency response notifications were made to the Green River Fire Department and FMC requesting assistance. The fire was officially declared out at 3:20 PM.
Roof failed at xcut #23 in return on #3 beltline. The fall was a continuation of a previous fall. Unintentional roof fall.
HMI failure at #3 Hoist, would not reboot completely, hoist would not operate consistantly. Started evacuation through #1 shaft. Rebooted HMI, PLC and DHS hoist did reset, but functionality was inconsistant still. Changed HMI computer unit function was restored, test trips and return to service at 04:45a.m.
#2 hoist conveyance did not fully open at dump. Conveyance became lodged in shaft dump area. Very minor damage to the skip door was incurred. Crews were removed to the 900 level where there is secondary egress. Skip was handmucked and is under repair. Secondary egress was re-established via #1 skip within 3 1/2 hours.
The mine encountered an unintentional roof fall on the Main South, entry #4, at the 3+60 intersection. The fall measured 60' X 18' X 8' thick. It did not affect ventilation or the passage of men, no personal injuries. The area was developed on 6/16/07, supported with 8' point anchor bolts. This fall will not be cleaned up.
An unplanned roof fall was discovered app. 1,200 ft. from the active section 1 x-cut inby S.S.#6088. The fall measured app. 32' in length X 19' in width X6' in height. The area was bolted with 6' torque tension bolts. The area is dangered off and will not be rehabilitated. MSHA inspector investigated the fall.
The Moly flotation area was evacuated because two fixed H2S monitors were in alarm mode. It has since been determined that the monitors were alarming because of two separate smaller events rather than one large event.
Unintentional Roof Fall 5' x 18' x 80' above the anchorage point of the 4' foot fully grouted bolts in the no.1 left return entry at spad 260.
A section of roof measuring 20' w x 10'l x 8' thick fell at the highwall over #4 intake fan entry. The rock damaged the fan housing, interrupted ventilation and blocked the primary escapeway.
While making the required checks on the safety catches of the Page Service Elevator, one of the spare counterweights stored at the coal seam level was accidently knocked into the sump of the shaft striking one of the tail ropes resulting in broken wires & distortion. No one was injured & no other damage was reported. The damaged ropes were replaced.
An unplanned roof fall occurred in the # 4 entry at survey station #5797 appr. 3,000 from the active section. The fall measured 22' in length X 20' in width X 6' high. The area was bolted with 6' torque tension bolts and has been dangered off. The area will be rehabilitated.
During the morning startup the production hoist kicked out. It was reset and moved, but then kicked out again. The hoist then could not be restarted because of an overloaded motor lead. An evacuation was called at 5:25 am because of loss of the secondary escapeway. MSHA emergency notification was called at 5:29 am.
While doing retreat mining on block #316, one block inby spad #1852, the continuous mining machine had a roof fall on it before setting it in the clear. It covered approx. 20 feet of the machine, 10 feet high.
This mine experienced a roof fall in #1 entry at the 5 + 30 cross cut.
An individual was entraped on the elevator at the preparation plant between the 6th and 7th floor for a period of timie exceeding 15 minutes. a blown fuse was found and the elevator was repaired to get the individual out. Communications was established and continued. There were no injuries and all agencies were notified according to law.
The #6 portal elevator became inoperable when the manual break caused a rise of temperature on the amperage on a motor. The elevator was returned to service after dirt in the manual break was cleaned.
A fall of the roof occurred on the old #5 unit. Fault project suppy rd. Length was 25' by 19.5' in width by 10' in thickness. Personell, equipment and ventilation was not involved. X-cut 655.
Operator of a Getman scaler started the unit and noticed dense smoke immediately. There were no visible flames but the smoke was intense so he evacuated the area and notified supervision who in turn evacuated the mine.
A roof fall occurred in the No. 5 Entry of the 3rd Panel South off the 1st Main East. The fall is located at xc 13 (spad 2+31) andis approx. 10' high x 18' wide x 100' long. There were no injuries, witnesses or disruption to ventilation.
Roof Fell at spad # 873. Fall was about 20' X 20' X 6' thick. Fell up to the coal rider.
Drilling holes with a highwall drill to prepare a shot when one of the holes started having smoke and heat coming out of it. Could not extinguish in 30 minutes.

Contractor shift supervisor noticed increasing water at the 700 sump and he contacted the mine supervisor. The mine supervisor determined that water could fill roadway and hamper normal vehicular traffic on secondary escapeway. Crews evacuated the mine while the water level was lowered by pumping.
A roof fall was discovered on the slope track as persons were traveling into the mine. It was about 10 ft. high, 50ft. long and 24 ft. wide.

Found a fall 5 break off section in #5 entry 1 break outby spad 2123. An intersection fell in about 6 1/2'-7' thick, 25'-30' wide and 25'-30' long. Fal lwas cribbed and dangered off from all sides.
Roof Fall date unsure, sometime between 07-01-08 and 07-07-08. Located at intake airway #9 (2200' from face) 10 x 17 x 15.

A roof fall occured 3 Main 9+2 10x #3 Hdg. Area was bolted with 6' fully grouted bolts. Fall was above anchorage, 30' long rib to rib. 4,200 ft outby section. No persons or machines were trapped or injured. No ventilation or travel were affected.
An unintentional fall occurred at the 6+30' in #4 entry in 2nd West sub main. Fall area was bolted with 6' tension rebar bolts and 10' cable bolts had also been installed as supplemental support. Fall was 20' long by 70' wide by 11' high.
Tailgate blockage 12 west LW MMU-010, floor heave and broken cars in travelway. #3 entry of 11 west inby end 35+00, outby end 31+00 (21 1/2 x-cut to 19 1/2 x-cut).
Roof fall. Fell on off shift. No one underground at time of roof fall. Inby survey station 17 & No. 3 line of crosscuts. In entries #1 to #5 entry.
A roof fall occurred in the #9 seam in the 1 North at crosscut #70 in the pillarline between #6 and #7 entries measuring 35ft. x 18ft. x 5ft.-6ft. high. This area was supported with 5 foot fully grouted roof bolts.
#3 Hoist computer allowed the hoist to overspeed to more than 300 feet per minute total. The hoist safeguards immediately stopped the hoist. The computer was reset and the fifteen employees within the cage were brought to the surface. After successful function testing the hoist was returned to service.
A roof fall was discovered in the No. 2 Bleeder Entry, one break outby survey station No. 2165. The fall measured approximately 110' long by 20' long by 20' wide X 12' thick and fell from the pillared area into the intersection of the No. 2 Entry.
There was a roof fall discovered on south 2 section at break 72 between #1 and #2 entries. Measuring approximately 80' L x 20' W x 8' H.
A roof fall was DISCOVERED at 8:40 a.m. in the NO. 2 Bleeder Entry one break inby survey station No. 2201. The fall measured approximately 70' long X 20' wide X 5' thick. The roof fell from the pillared area into the intersection of the No. 2 Bleeder Entry. The fall occurred approximately 1400' inby the active section.
Roof Fall. Fell in the intersection of the last open break in #2 entry. Fall was about 20' X 25' X 6' thick. Fell up to the coal rider.
East skip door would not close after loading skip to haul muck. It was discovered that the kicker rail had broken loose and become hung in skip. This caused the hoist to be unusable for more than 30 minutes.
Slurry is entering the streambed at 3 locations approximately 1/4 mile downstream of the dam. The flow rate is estimated to be 600 to 800 gallons a minute. The flow is contained and being pumped back into slurry impoundment #5. MSHA has done a full investigation of the accident and has released the impoundment for work to resume as normal.
Roof Fall #4 entry spad # 860. Fall was about 20' X 30' X 5' thick. Fell up to the coal rider.
While repairing a crack in the revolving frame on the dragline a contractor accidently cut through the floor. Sparks ignited grease below. He attempted to extinguish the fire but was unsuccessful. It was reported to MSHA at 1:45 am. Due to the inaccessibility of the fire, it was declared extinguished at 5:00 am. No injuries resulted from the incident and fire damage was minimal.
A roof fall occurred 10,600 feet outby the working section in the #6 entry near spad # 485. The fall was 40' long and 18 feet wide and 5 feet thick. 4 ft 3/4 glue bolts and 12 ft. cable bolts had been installed in this area. This was a transitional area from sandstone to shale.
A fall of roof occurred at the intersection, in # 6 entry, cross cut 26 in the South West Main. The fall was approximately 25 feet in length x 25 feet in width x 10 to 12 feet thick. The area was developed in April 2004 and had been supported with 6 foot fully grouted roof bolts.
A roof fall was discovered in the 2 north 3 mains, 18 crosscut, XC1-2. Contiuation of existing fall, measures approx 18' wide and 8-10' thick. The area is supported with 5' fully grouted resin bolts and 15' point anchor bolts.
A unintentional roof fall occurred on the 5th Southwest, S.S. 9+90, Ent.#4. The fall measured 40' X 19' X 8', The area was bolted with 8' point anchor bolts on 4/23/07. The fall did not effect ventilation or passage of men, no injuries or property damage. This fall will not be cleaned up.
Man loading dump truck from stockpile. Did not set parking brake or chock tires. Truck rolled down hill 300 ft and over 10' embankment.
Breaker tripped causing loss of power to the elevator.
Loss of power to primary hoist at 5:30 am. No damage was done to the hoist power restored at 6:10 am to the hoist.
Employee was marking locations to install roof bolts, a piece of slickensided rock fell striking the employee's head and crushed his head against the drill station canopy.
Unknown, most likely human error. Employee passed away on 7/25/08.
From accounts of person involved in accident, EE was preparing to tram Feeder onto belt tail during 3 East belt move when power cable for Feeder was dropped. It appears it fell across tram controls undetected. When EE activated start control switch, Feeder Hydraulic Tram Circuit engaged causing it to tram toward him, striking him & pinning between Feeder & coal rib.
Belt foreman was jacking up tailpiece to put a header board under it. He was cleaning out under the tailpiece foot when the jsck kicked out, let the tailpiece drop down.
When using an industrial kiln gun to shoot ash ring in the kiln a shell exploded injuring the right eye. As a result of the injury the employee lost his right eye.
Employee tried to dislodge stone from conveyor while machine was running. Lost part of her pinkie finger.
This victim was liftng a final drive for a scoop into place by himself. This was too much weight for one person ? resulted in a hernia. Was placed on light duty until September 13, 2008, when surgery to repair hernia is scheduled. No work days missed at this time.
Employee got his index finger on left hand caught between canopy of scoop & rib causing the finger to be amputated to second knuckle.
Employee stated that he was laying on his back installing cap bolts on dragline boom when he felt a burning sensation. Dr. diagnosis was an umbilical hernia and assigned employee to restricted duty.
The employee placed a clinker sample ladle under the head pulley of the #3 kiln transfer pan conveyor to obtain a sample. The ladle was caught by the conveyor severing the employee's right hand ring finger between the ladle handle and the framework.
EE was tramming a double head roof bolter up a hill and the roof bolter had to be pushed by a scoop. He accidently stuck his foot outside the operator's compartment and the scoop bucket hit his foot. The injury to his left foot required that the ends of his 3rd and 4th toes be amputated and the 2nd and 5th toe was fractured.
EE was in the proximity of the power center when the power plug flashed burning right hand, arm and neck.
Getting on the excavator, put too much weight on left arm.
Avulsion of the left index finger to the first joint by removing a drill steel under lateral pressure.
Employee was spot bolting top in an outby area. A rock 23"" x 26"" x 6"" thick fell from the rib hitting him in the back.
Injured was removing a piece of rock from the shield skids from a crouched positon. The rock weighed approximately 30lbs. Knee pad slipped causing a twisting motion causing pain to groin area. Employee worked 11 days and went to the doctor AUG. 13, 08. and was diagnosed with a double hernia.
The employee was grading the mine floor with a Joy CM1415 in the 2 Lt. #3 bleeder entry. In the process a piece of roof rock fell unexpectedly between roof bolts and struck the injured on the left shoulder and back. The average height of the No. 3 entry is approximately 9.5'. The employee started missing work on August 4, 2008.
Employee was replacing an air compressor on a Mack water truck. The compressor mounts in close quarters, and slipped, mashing his little finger against the truck frame. The little finger was cut, requiring 3 stitches.
Employee was installing elec. underground. When he was getting out of trench he twisted his left knee.
After staging truck at loading face the right rear trailer strut failed as the 1st bucket load of coal was dumped into truck. The resulting severe rocking motion slammed the driver into the operators door of the tractor.
Upon an investigation the employee dismounted/abandoned a moving manrip when returning to the portal ending his shift. A scoop blew a tire that erupted in smoke that caused panic and fear. The event resulted in a lost time accident.
Miner was shoveling under belt. Shovel contacted belt and whipped back across left knee. Bruised knee resulted. Miner submitted injury report to safety office on 8/15/2008. This 7000-1 is a resubmission of a report form submitted 8/15/2008.
992G loader operator mistakenly hit the quick drop on loader while reaching into floor board area to pick up a snuff can. The boom came down on the side of the truck jamming the truck driver; driver felt pain in back.
After loading the 906, employee was chaining the loader down. After he had the one side done he moved to the other side. While moving to the other side he was in between the truck and the goose neck. As he was coming out the other end he stood up too soon and hit his left shoulder blade on the gusset plate that is on the trailer.
This miner was pulling a cable, when he stepped in a hole and twisted his right knee.
Employee was hooking duster to a scoop and felt a pain in his right knee.
Employee was pulling cable to the face. Employee looked through the curtain which was on the horn of the miner. The curtain caught the control levers causing the miner to hit the employee's knee.
Riding on mantrip going to the working section and the mantrip hit a bump in the road causing the employee to hit his head on the mine roof. This was not reported to management until 8-4-08.
Two employees were attempting to move a scale using a forklift and by hand and one of the employees put his hand in a pinch point area and injured his finger.
EE was removing a roof drill steel out of bolt hole and felt a pain in his groin.
Employee stated that while lifting a jack he hurt his back.
Employee was operating a D10 dozer feeding a sand & gravel operation. A rock unseen by the operator rolled behind him and he backed onto it.
Injured was lifting timber when he felt pain in groin and back area.
Worker was re-entering a stockpile truck. He wrapped his right hand around the door frame to close the door. He slammed his hand in the door.
Employee was cleaning under 5 pound line when a dust shield came off. He tried to put it back on and got his left hand caught in belt.
Employee was helping the crew run a new electric cable down the bore hole into the mine. The cable restraining device came loose and caused the cable to whip and strike him in the leg.
Stepped out of guard house onto pavements twisted left ankle on his way out to do foot patrol.
Member was taking lunch break, he was standing beside a service scoop when the rib rolled off and struck the Member in the side, knocking him into the scoop. Member was later found to have a fracture to the pelvic. This accident was called into the MSHA hot line when the fracture was discovered.
While working on belt structure, EE slipped and fell twisting EE's left knee.
Employee was walking and tripped over hole in mine. He slipped and sprained his knee.
Employee was cleaning tailpiece and the anchor pin pulled loose and hit him in the leg.
Injured EE was in the process of removing a cover lid from a continuous miner to place on mine floor, when EE stepped on some loose bottom, resulting in EE spraining his left ankle. Injured EE continued to work until 12/22/08, when his family doctor removed him from work.
Employee was dragging loops of cable of a continuous miner around the corner of a coal rib and strained his back.
Employee was loading belt structure, twisted and felt sharp pain in lower back.
Miner was using a scaling bar to knock small rocks through the welded wire mesh. He bent over to pick up the scaling bar and noticed pain in his lower back.
Employee noticed anti-freeze around the radiator cap when he went to tighten the cap it popped off and sprayed anti-freeze on his back and arm.
EE was walking and stepped on rock and twisted his ankle. Went to DR on 7/30/08 and was told to take the rest of the week off. EE will miss 3 days & is expected to return 8/4/08.
The employee was transporting a load of refuse from the new refuse bin to the 1050 bench site. While driving to the area, the 777 haul truck hit a hole, causing employee to bounce up and down in his seat. He began to feel pain in his lower back soon after.
He was removing an auger from the auger sled and the next auger dislodged itself and roll off hitting his ankle.
Had a set of come-alongs hooked to dolly on the belt takup. Dolly was picked up. The hook broke allowing the come-alongs to strike the injured on the right leg and thigh. He was taken to Wlch Emergency Hospital where he was diagnosed with a deep bruise. Treatment is several days off his feet.
Three inch water pump had been cavitating causing the water to heat up inside the pump. When the hoses were disconnected it sprayed hot muddy water which caused first and second degree burns.
Employee bent over to lift a planitary cover off of a 988 loader when he felt a sharp pain in his low back & leg.
EE was cleaning off the top of the shearer, when he pushed against a rock he felt pain in his right shoulder, he continued to work at his regular duties but contined to have pain. He was removed from work on 12/3/08 for surgery to the right shoulder to repair a torn rotator cuff.
Employee knelt to position body in order to hang curtain.
EE had been helping grease his shuttle car and was walking back to the operator's deck. He slipped and fell and his arm went in between the discharge roller and the rear brace. He bruised his ribs and stained a neck muscle. EE continued to work through 9/11/08 and his physician took him off work effective 9/12/08.
Trying to get the drill steel unstuck, he got his left middle finger caught between the bolt head and the drill pot.
Injured was cleaning walkway and picked up rock straining groin area.
EE was pulling slack off the roof bolter reel and slipped and fell to the ground. He landed on his light battery, he finished bolting the area and then stopped work and stated he felt pain in his left side and rib cage area.
Employee was preparing to put oil in his equipment at the start of his shift. He decided to turn on the headlights because it was still dark. After coming down off of the piece of equipment he took a few steps and stepped onto a rock, turning his ankle.
Backing up dozer, ran over wind-row of dirt & jolted body, injuring neck.
Injured was bent over when he lifted the cable up his chest popped and took his breath. A knot came out on the lower part of his chest. He went to the Doctor and was reported as being off Aug 1, 2008 for the accident.
The EE had just finished his pre-shift when he was getting off the 990 loader,slipped off top step and fell down 5 steps to the lower level. He hit his left shoulder and backside. It wasn't a lost time accident or reportable until it was determined on 8/8/08 that he had torn ligaments in his left shoulder - determined by panel doctor.
Miner was removing covers from the section continuous miner and felt a pain in his chest.
EE was picking up a rock that had fell on the H20 line and strained his back.
Miner was walking stepped on a plastic line and turned his ankle.
Shoveling material for approx. 45 min. twisted left to right and felt pain in lower back.
Employee was walking in #5 entry, 3 north left section and slipped on uneven bottom and twisted his left ankle resulting in a fracture.
Employee was operating a water truck and was walking to the pump when he lost his footing and fell. He jammed his shoulder upon impact with the ground. He initially felt only a slight tingling sensation in the shoulder, but upon arrival at his home, he was unable to elevate his arm.
Employee bent over to pick up a crib block to move out of the way. Shortly after throwing block out of the way, the employee started complaining with neck pain.
While steam cleaning oil cooler on hi-lift, the employee was walking on the catwalk and slipped. He reported that he caught himself and felt something pull in his shoulder.
EE was starting to drill the inside hole to install a roof bolt and the drill steel bent and struck him on the ritght shoulder.
EE driving shuttle car to miner and ran into a dip and hit head on canopy of shuttle car.
While carrying a longwall part (slow feed) with 4 other crew members, employee's foot slipped in mud while setting part on pallet and pulled muscle in lower back. 12 west 1 to 2 entry and 19 x-cut.
He was standing in roadway waiting on scoop when a piece of gob fell out hitting him in the head.
EE was roof bolting when a bolt strap fell and cut his right forearm. Resulted in 7 stitches.
Victim was checking the differential oil on a 992-G loader. When loosening the fill plug, he slipped and hit his left elbow on the rim of the loader. Sustained a fracture to his left elbow. Missed 4 days work: returned 8-2-08
Stepped wrong and hurt lower back while carrying support for conveyor leg.
He placed his hand on a loose piece of rib to pull down, causing the rib to slide down against his ankle.
While helping to remove cross over walkway off belt line, his left arm was caught between the walkway and belt chain, causing a contusion to left arm. MRI on 8/15/08 was inconclusive and he was removed from work and placed on physical therapy for 2 days a week.
Employee states he was walking along the 3MN conveyor belt storage unit when he stepped on a piece of coal or rock and twisted his lower back. He continued to work; however, due to his back pain, was unable to work starting on August 8, 2008.
The employee had just completed changing out a welding oxygen cylinder. The employee was placing the empty cylinder in the back of a jeep. He bent down placed his left hand on the bottom of the cylinder; when he lifted he heard a pop and had instant pain in his left bicep. He was taken to the ER on the 27th, a specialist on the 28th and is scheduled for surgrey on the 29th.
Alledged incident was to have occurred while this employee was handling 5-gallon oil cans, he claims to have heard a ""pop"" in his right shoulder. Cause if injury if occurred was overexertion. Compliance with rules/regs was not a factor, protective equip not a factor, miner proficiency a factor, mining equip/systems not a factor
Employee was getting into the operator's cab of a cherry picker and banged his knee. This resulted in a torn muscle and lost time.
While chocking railcars he got his finger and thumb between the chock and the railcar wheel. No bones were broken but the tips of the middle finger and thumb on the left hand has tissue damage on the tips.
Employee was tramming diesel scoop along 1 North roadway when he ran over rock in roadway & jammed neck into canopy. Employee continued to work until taking off beginning on September 2, 2008 due to pain. This accident became reportable on 09/02/08 due to missing work.
Employee was removing a bearing from a centrifugal dryer when the bearing slipped, employee caught the bearing resulting in a cut that required stitches.
EE was using a slate bar to pull pin from a rubrail on C/M. When the pin came out, the rubrail struck him in the bend of his leg (ankle/foot), knocking him to the ground. He was complaining of pain in his right leg, foot/ankle. The rubrail was bent just slightly making the pin difficult to pull.
Employee was unloading sulfuric acid out of a semi tanker into a storage tank. He forgot to check to see if a gasket was in the coupling device and when he opened the valve to unload, acid sprayed out onto his PPE with a little liquid running down his right sleeve into his glove causing a burn on his right forearm.
Tie rod boke on 60 ton euclid haul truck. Truck struck berm and came to stop.
Taking transmission filter off of 988H. Spun the filter off and fell into bucket and pinched finger. Worked his full shift on 7/26/08 came in on 7/28/08 worked a couple of hours and went to the doctor. Took off 7/29/08, came back to work on 7/30/08.
While roof bolting, coal rib rolled out hitting employee in back of legs pushing him up against the tray on the roof bolt machine. Employee hit the tray with his right rib area.
Operating scoop had hand laying on top of panel mashed finger between panel and frame.
Fitting broke on the steering valve spraying hydraulic fluid on the turbo which ignited. Employee then jumped from the truck and received 1st and 2nd degree burns to his arm and ear. No damage was done to the truck.
Employee was re-installing a suspended fan to a bulkhead when it moved and contacted the employee. Employee continued to work with a full release from the Dr., until 8/01/08 when work restrictions were imposed.
Employee was walking up toward face #2 near corner of 2 right, one cut inby spad #170. The shuttle car was in face of no. 2. The shuttle car started to take up cable as employee straddled the cable and the cable struck him on the inside of his right leg just above the ankle.
""I lifted a rock and pulled a muscle in my left knee. It felt like a cramp. I thought it would work itself out but instead the pain began to travel up into my back as the day ended. When the injury incurred, I told 2 co-workers. On July 28, 2008 I saw Doctor. He checked for inflamed muscle and took me off work.""

EE worked untill 7/26/08 at that point he saw a DR who told him to remain off work untill 8/1/08 due to an infection in his knee from falling onto a rock.
Employee was working on crusher deck which is 3 feet off ground; he went to grab a bar, slipped and landed on the ground on his feet. He then stumbled backwards and fell on a piece of steel on the ground.
While replacing a cover on a miner, the employee caught his gloved left middle finger between the cover and the frame resulting in a broken finger tip.
Injure was riding the outby end of a 13 man National Mine Service mantrip. The handle of the sander came in contact with the top causing the plunger in the plastic sander jug to break free, striking him on the left side of his jaw and neck. The height in that area was 44 inches from top of rail. Canopy measured 36 inches.
A hydraulic failure of a coal truck spill protector allowed the spill protector to contact a personnel lift and push it over. The employee was wearing a safety belt in the personnel lift. The lift tipped over and the employee's leg was fractured when the lift contacted the floor.
Employee stated that he was getting out of truck when he missed a step, falling to ground on right shoulder. ""Report filed under protest"".
Employee was on 992G when he fell from the fender hurting his left arm.
While roof bolting the employee was changing the bit insert when a piece of rock fell from the roof striking him in the back.
Euclid lost it's brakes as it was going up the ramp and started rolling backwards. It hit th ebig rocks being used as a berm. A tire blew out as Euclid hit another rock and came to a stop. Driver was still in the vehicle, turned the motor off and put it in gear. Got out, walked up to the top of the quarry, then was taken to dr. Only bruises shoulder/leg. Was wearning seatbelt.
While making a belt splice on the Horse Creek # 2 Belt, Injury attempted to move the nailing plunger, causing the person doing the nailing to hit his hand with a hammer causing a fracture to his right index finger.
The employee was working in unison with the scoop operator performing rock dusting procedures. The scoop was backing into the 12 x-cut right to pick-up some dust when the employee placed his person near the corner of the inby rib and his foot was caught between the rear scoop tire and mine floor.
The employee was walking to the section when his knee became irritated and painful. He has had problems with his knee popping out of place in the past. On 10-08-08 the employee had surgery. MSHA lost time accident 10-08-08.
EE suffered a left middle finger injury. Went to DR on 7/25/08, diagnosed a fracture finger.
While being loaded from a shovel a large rock rolled off the shovel and jolted the truck, causing ee to hit his knee on the retarder lever. Employee did not start missing work until 07/28/2008

EE was removing drive mounting bolts.When rmoving the 7th of the 8 bolts the remaining bolts sheared off.The stored energy in the drive caused it to rotate rapidly.The belt guard on the drive struck employee in the left shoulder-throwing him forcefully into a handrail and structural beam.He was evaluated by a DR and taken off of work.
Employee jumped from a rear ladder of a scraper while starting the rear engine resulting in a suspected injury to left knee.
EE was squaring the end of the belt for a splice. He was trimming off a piece of rubber, when his knife slipped, cutting his right leg above the knee. *Note: We were 1st notified of this incident by mail on 8/5/08 form Workers Comp. Then I received a report from the mine on 8/6/08.
EE was getting tools from side box of truck. He placed tools on ground, shut box and thought he had the box latched. He picked the tools up, stood up and the tool box door fell open catching him across the forehead just below the brim of his hard hat, cutting his forehead about 2 inches long and requiring 5 stitches.
While greasing a 992 loader, the grease plug came out and struck employee on the cheek, below eye.
EE was driving a pin on the chain conveyor when a piece of metal flew off and struck employee in the left hand, between the 3rd and 4th finger. Employee did not miss work or recieve medical treatment until 8-7-2008, when the metal was removed.
EE was walking across the track and stepped on a rail which caused him to twist his knee, he continued to work untill 7/31/08 at that point he was told not to work by DR.
An employee tripped over the pump drive room door, striking the lower right side of his back on the stairs. The employee was placed on resticted duty making this a reportable.

The injured employee and a Motorman put an empty supply car on the Contrary Service Cage in order to send to the surface. When the injured uncoupled the car and flagged the Motorman to pull away, one of the safety chains caught the horn on the motor coupling, which pulled the supply car on the right leg of the EE, resulted in fracture of the right tibia and fibula.
EE was lifting around 20 lbs tray for a salad bar. While lifting, she twisted her lower back, lower back muscle stain. She was distibuted with 2 doses of 30 mg of Toradol on site. Rotational s chedule, RTW date is 8/6/08.
Employee was working with another man roof bolting on a twin boom bolter. The co worker moved the machine over without warning and caught the injured man's left leg between the bolter head and rib.
Employee stated that he was standing on a rock nailing the line curtain to the rib in preparation to scoop and rockdust the place, when he slipped off the rock and fell striking his right knee on the mine floor.
Employee was stepping off of a 330 excavator during a rain storm. When his foot made contact with the gravel his foot slipped, causing all his weight to rest on his right foot resulting in a sprained ankle.
Employee got something in his right eye while pulling the curtain
A forklift operator was placing a load of boring equipment onto a tractor & trailer. While loading the boring equipment, the container was bumped and a 2 1/4 inch bar jumped out of the cradle and fell from the trailer. The employee on the trailer jumped down when the bar fell and the bar bounced and hit the employee in the lower left leg, resulting in a fracture to leg.
EE driving Euclid R60, bringing sand to asphalt plant. After dumping, he heard a noise coming from left, rear tire area. He exited cab and saw a bulge forming in tire, turned to get away when tire burst. Was 10 to 15 feet away when tire burst forcing him to the ground breaking lower left leg. Another employee in area when tire blew suffered a ruptured eardrum.
As EE was walking along the up-hill side of the roof bolter, the rib sloughed off and struck him in the right ankle, resulting in a broken ankle.
He reached down to pull up miner cable and felt a pain in his back. This did not become reportable until 7-29-08.
Removal of an empty counterweight carrier from a D11 rcd tractor was found to be unbalanced which caused the front to be heavy when pins were removed. Anticipating the rear was heavy, a restraint chain had been installed on rear to prevent tipping but should have been on the front. Victim's right lower leg was momentarily pinched between crawler track & the brace of the carrier.
Employee was hauing shot rock to crusher from pit. Employee went to wipe sweat off his glasses and ran off the road. Truck went up berm and turned over on passenger side.

Employee was using a long bar to unlodge rocks in a transfer chute from the primary plant's scalping screen. Employee reported feeling a strain or ""pop"" in neck or upper back & the feeling of arms & legs going numb. Contacted EMS & transported employee to hospital for evaluation. MRI revealed no deformity & released him pending further evaluation by specialist.
Employee was installing a 40 ft long - 6"" diameter alum. pipe across length of sump. He was raising one end of pipe to top of high wall in sump, while he was standing on loose rock that was wet. His foot slid and he received a slight muscle sprain to his neck muscle.
The injured employee and another employee were carrying a piece of angle iron. One employee dropped his end of the iron causing it to cut the other employee's hand. The laceration required stitches.
EE was operating left side of the roof bolter on the 12 Right Section in the # 1 entry approx. 50 ft. inby survey station 25484 & as he walked towards the end of the roof bolter a rib measuring about 76"" high x 84"" wide by 26"" thick rolled off behind the line curtain striking the employee against the bolter resulting in fractures to left shouder, ribs, sternum & back.
Employee states he strained his back (aggravating an on-going condition) while loading bags of sand into a portable sandblaster machine.
Employee hurt his shoulder on 7/21/08 but continued to work. He went in for surgery on 9/2/08 and his first day away was 9/3/08.

Employee was cleaning up material spillage from under feed hopper. He glove got caught on conveyor belt roller. His left hand/forearm was pulled into and caught between conveyor belt and roller causing fracture and contusion to left forearm.
The miner was going underground, operating a scoop. The scoop hit the outside canopy, the miner slid over and hit his neck.
Employee was servicing a trackhoe, when he climbed off the ladder stepping to the ground he twisted his left ankle.
EE was hanging curtain in the #5 entry and hit his right index finger with the hammer.
A rock fell striking EE between his shoulder blades fracturing a vertebrae.
Miner was re-welding cracks on the rotex frames and at some point a piece of the metel got in his eye. He was not aware that this happened until he arrived at home and his eye started to bother him. He went to the ER and they took a small piece of metal out of his eye. He was wearing the proper PPE during the welding.
The employee was dismounting the new belt drive-head assy. for the no. 3 section and twisted/strained his left knee.
Injured was helping install arch roof support when he strained groin area.
Extension cord was run from laboratory through hallway and plugged in by water cooler. A large floormat was placed over the extension cord to use for a safe walking path. The technician walked around the floor mat where the extension cord was exposed, stepped on extension cord, rolling her foot. Technician fell backwards hurting her back and bumping her head.
Hose was buried in mud, the employee tried to pull out the hose in the mud, felt pain in his left shoulder. Sought medical treatment 7/20/08. Physician has taken him off work at this time until MRI can be done.
Driver, who works for another Savage operation, was at coal terminal unloading coal. She exited truck to check fuel level in left side tank. Driver removed fuel tank. The fumes ignited, burning the driver's face and right hand. Off-work, still in treatment. Cause still unknown.
Employee was applying light oil to the dollie rollers on #5 dragline. He was using an oil can with an approx. 18"" spout. He was standing on the shoe and leaning over to reach the rollers while the dragline was swinging. He then fell between the dragline shoe and tub, fracturing his pelvis.
Reported: Operating a 14CM15 continuous miner cutting a crosscut.As he was sumping in the left side of the cut, the bits began to strike sulfur balls and spark. He backed the machine, lowered the cutter drum and again sumped into the cut. A foreign object flew back striking his right forearm resulting in a puncture wound to the arm.
While putting car on, EE was putting miner cable in trough of Addacar. Car dropped off pad and his right foot was smashed/crushed under car.
Employee was working on a manlift basket. Employee was trying to free up the lock mechanism. He had his hand on the basket handrail and when it freed up, it dropped, pulling the employee off the CAT.
Employee was throwing yield props across the belt line and started hurting in his stomach. (Umbilical Hernia)on 06/20/2007, he was operating a tractor and hit a bump, had pain in his stomach. (Umbilical Hernia again) This type of hernia is a fatty membrane thru the abdominal wall because of a weak spot from the umbilical cord during fetal life. This is an Illness not a injury.
The employee stated he was dragging a piece of belt to get it out of the walkway and felt a pain in the back.
Employee was drilling in the 4R cross-cut and a piece of draw rock fell out hitting him on the hand and arm.
Injury was to have occurred while he was bending over to pick up a water hose on the floor but did not get to pick it up, he claims his back went out. Cause of this injury was overexertion and aggravation of old back problems. Rules/regs was not a factor, protective equipment could be a factor (back support), miner attitude a factor, mining equip/systems not a factor.
The employee was lifting and handling rock dust (40 lb. bags) and began to experience swellling in the groin area.
Original E-Document #640391 was amended in error on 8/22/08 as it was thought to be recurring injury. However, it was found to be pre-existing condition per MSHA definition. Therefore, this document is submitted as a separate incident.
While pouring concrete, the injured person got concrete inside his rubber boots causing concrete burns to both lower legs.
EE was getting out of the 933 skid loader and got right index finger stuck in the door handle that is located on the window. The window of the 933 is the door. Employee was opening the door and getting up at the same time. Finger got caught in the latch and twisted finger on handle before letting go. Door is hard to open.
The truck driver slipped and fell into a ditch that was being excavated from which he was hauling away the materials being removed to make the ditch, dislocating his left shoulder.
While changing crows and retract ropes on a shovel.EE strained his bicep while lifting the rope to reset it into the drum. Upon further exam it was determined surgery was required to repair the damage to the bicep. The EE worked without restriction until his surgery on 9/17/08, he was unable to return to work on his next scheduled shift on 9/18/08.
The injured was operating a scoop and was in the process of pulling into a crosscut to get some boards to build cribs with. As he drove the scoop into the crosscut, he ran over some loose roof bolts and one of the bolts came up into the operators compartment and struck him in the mouth. The incident cut his lip and broke off four teeth.
While installing drain pipe for rake thickener, contractor was pushing pipe to line it up when he lost his footing and fell from a retaining wall. Contractor was not wearing appropriate safety gear, although it was present.
After working for our company for only 3 months, the employee claims lower back problems from work done several weeks ago.
Employee walking across pit and was struck by a small rock falling from the high wall. Employee initially returned to work without restrictios. Employee began to experience pain in shoulders and neck approximately 4 weeks later. Employee started physical therapy on 10/23/2008.
Was trying to dislodge rock in wash box reject chute with a slate bar when his hand hit the edge of chute opening causing a laceration on his finger requiring stitches.
Waiting on repairs being done to his dozer, employee started inspecting a ladder rung that had come off another loader. When he attempted to pick it up he felt a muscle pulling his neck.
While walking back longwall face, his foot caught the base lift jack of the longwall support causing him to fall. He used his left arm and hand in an attempt to catch or break his fall which resulted in injuring his left shoulder.
After work, leaving parking lot accelerated rapidly lost control of motorcycle fell on road.

Employee stated he was carrying a oil can around a ram car when he stepped on a rock and started to fall. He felt his foot and knee pop, but when he tried to catch himself, he felt his other knee pop. Had pain in both knees, but went to the doctor on the other knee. Did not miss work until the 7/25/2008.
EE was picking up pizza pans when a rock fell out of the top hitting him on the side of his hard hat and left wrist. The rock was approximately 25""x16""x1.5""
Contractor EE was helping unload steel from a flat bed trailer, when load shifted, EE got caught between two pieces of steel and a compound fracture of his lower left leg resulted.
While lifting/manuevering truck tire into place, employee strained shoulder.
Employee stepped off of dozer and pushed the arm of the dozer. Got pain in lower back. Employee said he pulled muscle and drove himself from the job.
EE unloading roof bolt plates from scoop. When he picked up the plates and turned, he felt pain in his back and left leg.
Operating heavy equipment - cabin cooling fan cataputed insect into left ear canal.
Injury was roof bolting when a section of drill steel fell out of the hole, striking him on the right hand. He had a knot on top of right hand and a small cut on the palm.
Trouble shooting a problem with the top sheeter on a bagging machine. Employee hit the reset button and did not know that other employee was behind gated area. Arm on machine hit employee in the back causing bruising. Two days lost time - back to work regular job.
EE was walking on graveled area, slipped and reached for a oxy-acent torch cart to catch himself and struck his forearm on the hose holder, causing a cut to his right forearm.
Employee was helping pour concrete, small amount got inside his rubber boots. Caused small burn to his foot/ankle and heel. First Aid was rendered on site at the time and sought medical attention on 7-22-08. Reliance Medical Group diagnosed him with 2nd degree burn to the right dorsal.
Emptying dust box and drill cable caught left foot, twisting left knee (drill being operated by a co-worker).
Employee was removing build-up from a pulley with a bar when the bar caught in the belt and struck his left arm.
EE was bringing ATV across pit to move water pump. He lost control of vehicle and it rolled over.
The man was pulling a cable when he tripped and fell. When he fell he struck his back against his self rescuer.
Drill helper was holding drill rod and casing sub below rod holder so driller could reposition drill head. Helper failed to release the rod/sub as quill rod was raised. Helper's hands were drawn into the rod holder causing lacerations/avulsions to both hands and broken index finger on left hand.
Injury was watching the miner in #4 entry on #1 unit cut coal when a piece of rock fell out of the roof between the rib and the pins stricking him on the right leg.
EE was carrying conduit and slipped on loose gravel, causing him to fall and injure his right leg.
EE was in process of installing/replacing bearing on shaft of idler roller (snub roller) near discharge roller when chain broke which allowed belt roller to fall, pinning injured's hand between shaft and bearing housing.
Employee was carrying bags of ANFO to drill holes on a quarry bench. He indicated the next morning that his lower back was sore but that the condition may be due to a previous injury with another employer.
While moving a water pump and switch box from one location to another in the belt entry the slack cable was used up. When he got to the end of the cable the recoil jerked back on his left arm, injuring the elbow.
After changing a phase monitor on a electrical cabinet and engergizing to adjust voltage, an arc flash occurred and injured his hands.
Complained of lower back pain on 7/16/08 when stacking standup broken stone. Went home at 1 pm. He did not go to hospital (but was encouraged to). Returned to work 7/17/08 & stated he was OK. Went to hospital 7/23/08 with complaint of lower back and leg pain.
Shut door on loader and thumb got caught in door. Stitches were needed.
Employee was loading belt structure onto a 4-wheeler when he experienced pain in his left shoulder. The accident wasn't deemed reportable until the employee began taking off work due to complications from this injury on Novemember 10, 2008
The employee was installing rib bolts in an area of 6.5 to 7.5 feet when a part of lower rib dropped out striking the lower leg and foot.
Employee claims he hurt his back while shoveling up rock at the crusher. Pain has persisted and the injury began a lost time on 9/4/08
While riding in the front of a 5 ton supply locomotive that attempted to stop on a slight grade, but started sliding & slid off the end of the track, hitting a parked roof bolter. He refused emergency care & transportation. When he visited the hospital he was referred to a specialist. On 07-30-08 Marfork Safety was notified he had been diagnosed with a concussion.
Employee was crawling under a dozer when he felt his knee ""pop out."" This is a reoccurrence of an old injury.
While getting tools from the tool box, the lid of the tool box fell down onto his left hand.
Felt pain in low back while shoveling belt.
EE was operating a roof bolter installing supplemental roof supports (12 feet long cable bolts) in an area outby the face areas. As he was using boom of the bolting machine to push the bolt into the hole, bolt caught in the hole bent and rotated, the bolt struck the EE in his right upper arm.
While running a miner, a rock fell and struck his left leg.
Helping replace #1 CM dryer chute 2nd floor of plant, chute caught on edge of hopper. Using #2 hoist to pull #1 chute, using slate bar to pinch chute over hopper, with pressure on chute it came across hopper, struck employee in face causing injury. Employee had scheduled days off and did not start to miss work until 7-21-2008.
Employee was working on the #34 shovel removing plastic sheeting used to cover equipment during an overnight storm. The sheeting had blown down during the night, and covered an open access hole on revolving deck. The employee stepped onto the plastic, and his left leg fell through the hole up to his thigh, causing a deep contusion to the outside left thigh.
Employee was dropping the boom on his roof bolter when he put his hand on the boom to take the steel out of the roof. His finger got caught between the pot and the boom causing a fracture.
Employee ran his dozer into a hole and hit his leg against the door. He refused medical treatment at this time, stating he would be o.k. On Aug. 25, 2008, he went to the doctor with swelling on the knee and received 5 days light duty. He returned to work 9/3/08.
While carrying a jack along 13 belt, employee twisted his knee. After receipt and review of medical tests and records it was determined that this is a reportable incident.

Employee stepped out of grout truck, went down ladder and when he stepped off onto ground he twisted his right ankle.

On 7/15/08 EE along with other EE's were in the process of folsing belt for a belt move when the roof bolt eye broke causing the sheave wheel to swing down and hit EE in the right arm. Sheave wheel was being ised to assist in the positioning of the belt and was attached to the plate eye w/clevis.
Injured employee was operating a 10 ton locomotive on the outby end of a supply train. Employee became panicked while descending a slight grade and jumped from the moving equipment. When landing, the employee suffered contusions.
Member was lifting a piece of metal for building a rock box and felt pain in his stomach. A small knot came up. He went to the doctor and was diagnosed with a hernia. No surgery has been done at this time.
Carrying tool bags down beltline, he stepped in a hole and twisted his left knee.
Employee was roof bolting and as he attempted to remove a drill steel from the chuck, the top and rib let loose and he was struck on the neck, shoulder, and back by a rock 3' wide, 2' long, and 8"" thick.

Employee was dismounting a water truck. As his foot touched the ground, his ankle rolled to the side, causing pain to his right ankle.

Employee was descending down the ladder on a PC1100LC-6 excavator when he slipped hitting his wrist and hand on the tracks.
He was cutting weeds on the side of a hill and his right foot slipped on the wet mud and he twisted the lower part of his back causing injury to his lower back. He has had a history of back problems due to a past accident.
Employee was trouble shooting electrical circuits with meter when the meter failed causing flash burn to both hands, due to carbon tracking in the panel.
Injured employee stepped in hole near upsweep of fan.
Employee was lifting wheel off of a personnel carrier and strained his lower back and left leg. He began missing work on 8/13/08 to start physical therapy.
Employee was pulling (hanger) bars when he first felt the pain in his lower back. When he got on the forklift is when he was in a lot of pain. He was taken to the doctor, and taken off work for 72 hours, until seen by PCP. PCP extended the time off. Currently ONGOING.
Operator of shovel was loading large boulder onto rear end of haul truck(60 ton haul truck). The boulder slipped out of bucket and dropped into bed of truck. The bed raised up and then came back down with force causing whiplash type injury to haul truck driver.
While climbing down truck ladder, worker slipped and fell on the ground injuring his left shoulder

Employee was walking around foreman's pick-up when he walked into the trailer hitch causing him to fall on his left arm, dislocating his left elbow.
THIS ACCIDENT OCCURRED AS THE LABORER WAS ASSISTING OTHER PERSONS TO TIGHTEN AND ADJUST THE CONVEYOR BELT IN THE AREA OF THE HEAD PULLEY. AS HE WAS PUTTING PRESSURE ON THE BELT FOR SOME UNDETERMINED REASON HE SPRAINED HIS LEFT WRIST.
Employee had been loading material in a hopper with a front end loader. He walked up the ramp to check the level of material in the hopper. As he was walking down the loading ramp, he slipp ed on loose material. He braced himself with his right arm, causing discomfort in his right shoulder.
Employee stated that he was carrying a side cover for the Alpine miner (approx 2'x 2') in the track entry of 3B miner section. He stepped on a piece of coal causing him to turn his left ankle. This caused swelling and the doctor told him not to put on a miner boot.
Stepped off lube truck, rolled ankle on uneven ground.
EE had set up crane to lift bed off 777A truck Crane was set up perpendicular to the truck & 3' lower in elevation. Rigging was hooked to bed 3 times. As truck bed was swung, the crane tilted toward truck bed slightly. EE started lowering load & began retracting the boom. Crane stopped tilting, EE thought everything was fine, but the crane suddenly laid against truck bed.
Working on clean coal dryer basket using bar to pry basket, he felt something in his back give.
Walking from portable plant over to the finish plant.
Twisted left wrist. 2"" water pipe fell of sled, while trying to reload, he twisted his left wrist. Did not begin missing until Jan 28, 2009 due to surgical procedure to repair the damaged wrist.
Injured was unloading belt structure. Pushing from supply car onto scoop, strain of the right gastrocnemds muscle (leg).
Employee had 2 lighters and ""screamers"" in his pants pocket, one of which ignited while grinding. Employee claims screamers were spent already and that the lighter ignited. Employee received 3rd degree burns to his left leg.
Employee was driving a vehicle on a underground travelway when a rock fell from the roof and struck him on the head and in the face. He continued and struck a coal rib. He was transported to medical services.
Employee was working under temporary roof support with posts set. Two rocks fell behind the employee and as he went to get away a larger rock fell on his back and pinned him to the ground face first.
Using a hilti gun to hang canvas, a piece of rock fell from the strap and struck him on the left side of his head. He continued to work at his regular duties. He went to a physician and was taken off work on 8/8/08 due to a disc herniation.
Employee was a passenger in a vehicle that was traveling on an U/G travelway. A piece of roof rock fell and struck the driver of the vehicle causing him to probably lose control. When this happened the vehicle struck a coal rib throwing the passenger out to the ground causing a fracture to his left instep (metatarsal bones).
The employee was working at W 1 tailpiece. While shoveling, a piece of rock came back on bottom belt striking employee's right hand resulting in a laceration to underside of last digit on his middle finger. After being treated and returning to work, the finger became infected subsequently requiring the employee to begin to miss work.
Employee was walking through the parking lot, on his way to his vehicle, when he twisted his ankle. He went home and the pain got worse, he then went to the dr. and found out he had broken his ankle.
Roof bolting and a rock (4' x 28"" x 4"") fell striking employee on right shoulder, right side, and lower back.
Contractor mechanic was stepping from a D11 track to dozer arm to get down from machine when his foot slipped, causing him to fall to the ground and hit his shoulder and head. He bruised his shoulder (rotator cuff). No loss time-light duty.
Employee reported pain in R Knee after kneeling. Later employee bounced his knee into LHD controls after driving over a wood block. Report is filed at this time because FMC has just received medical confirmation that the injury could have resulted from this incident.
The miner was de-energized. EE was moving cables to prepare to cut a corner.He put the rope on the bolt on top of the J-hook and went outby the miner.He started the miner and the rope slipped off and he leaned in to pick the rope up and was pinned against the rib with the boom of the miner.He freed himself and walked to the front of the miner.
Employee was working on quarry water pump. He reached behind the pump to access bolts and felt a pulling in his groin. He continued to work until his surgery Aug. 26, 2008 when this became a lost time accident.
EE went to clean up under the hoist motor coupling and was going to give the motor coupling a quick wipe because it was a ripping grease onto the floor. It caught her glove somehow and pulled her finger into the bolt hole. She felt a tug and a sting and when she looked. the tip of her finger was gone.
The brake assembly at the Kiln Aux drive was disassembled as it was being repaired. The drive was started and the assemblies spun and hit the bolts that held the drum on. The brake assembly broke and hit the employee in the leg, who was standing approx. 12 ft away. This resulted in loss of time.
Employee was washing down some equipment with hot water and when he laid the hose down the water sprayed up his right sleeve causing second degree burns.
Employee stated that as he walked into the rectifier to perform a trolley check his earmuff caught on multiple cables causing pain in his neck. Employee did not begin to miss work until 7/22/08.
EE was welding some screws of a transmission in the conveyor of the plant #2 and when he was going to get up, he could not get up, feeling strong pain in the waist and the upper body.
Employee opening/closing bottom gates on rail car, right leg slipped, over extended knee, resulted in a torn miniscus. Classified as first aid with no days missed until surgery on 10/14/08.
Employee bent over to pick up a roof angle and back ""went out"".
While applying torque to a bolt, the drill chuck and motor fell off and hit him in the left knee, causing contusions to the left knee and left rib cage. He continued to work at his regular duties and eventually went to a physician. He was removed from work on 9/10/2008.
Loader operator felt slight pain in his knee while going up the ladder at the asphalt plant.
Wrecked manbus and he hit his right hand on the batteries while trying to catch himself. this accident was reported to me on 8/21/08 that why (late sending this reported in)
Moving miner, a loop of cable slack pulled across, striking him and knocking him to the mine floor.
This victim struck the rib while rounding a curve and caused the rib to roll out into operator's deck. Alleges a separation in right shoulder. Went to one doctor and was told he could return to work, so he went to another that is keeping him off work.
Caught hand on roof bolt and broke finger. *He started missing work on 10/23/08 due to having surgery.
Employee reported: doing work in shop area of plant, bent over to pick up a piece of steel plate, experienced pain in lower abdomen. Sought medical treatment a few days later. Determined he had a hernia and could need surgery. Surgery was delayed until July 30, 2008; started missing scheduled work as of that date.
Injured was attempting to free a stuck shuttle car using a scoop. The shuttle car slipped off the bucket causing the scoop to bounce, He hit his head on canopy. He was taking too Raleigh general hospital. Result x ray were negative but he was giving 4 days off and referred to family dr.
Employee was installing roof bolt, had placed drill bit against top, started rotation when glove on right hand got caught by drill steel twisting his right hand. Employee sustained fractures to his right wrist.
The was pulling flexible waterline under the belt when outby corner of coal seam rolled off striking him on the left leg.
EE was rotating a kiln using a ponymotor, 2 clutches are to be engaged. The 1st was engaged, EE was working at 2nd at the time of the accident. As the clutch was engaged, the pony began to labor and in order to avoid stalling, EE began to disengage the clutch. As he disengage it was backspinning driving the clutch plate in reverse, causing failure of the clutch.
Employee was watching as loader was backing up into #5 to #4 crosscut # North left section to srop off cable. When employee stepped back into a hump in the mine floor, he felt a pain in his right knee. Employee began missing work due to the injury on 8/18/08.
Shoveling material into wheelbarrow and hurt his back.
Employee was cutting trees, when EE was walking from one tree to another, he fell on his chainsaw.
EE was driving Cat 980B loader at top of stockpile. Skid loader died & rolled backward to sudden stop. He hit back of his head, got out of loader by himself but was dazed. Job foreman had him air lifted to Lubbock 602 Indiana Ave. Saw doctor and was released same day.
Employee was taking 970 into pit to empty out material when steam went into window of cab and burned his upper torso.
Employee was lifting a 5 gallon can of 320 oil to place on to the shearer in f-16 longwll section and felt a pain in the right side of his groin. Employee began missing work due to the injury on 7/17/08
Employee was shoveling belts, throwing coal over shoulder and caught hand in roller.
Employee was working with others moving a large generator using pipes as rollers; when he placed the pipe in front his hand got caught underneath, causing a crush injury to his left forearm.
Struck by 6"" chisel while spreading track links.
Employee was loading a part onto a truck. As the part was coming onto the trailer, he started to step onto the trailer. The lifting apparatus broke causing the part to strike the trailer causing him to be thrown to the ground.
EE was using a shovel to clean up a rock spill. He could not point to any particular incident that caused his injury but only had problems the next morning when he got out of bed.
While backing the roofbolter out of the entry, a cable bolt (8' long) was caught in rib on left side of entry and it flipped around and hit the employee in the face.
Injured was repairing a belt splice at #5 belthead. He was using come alongs to pull the belt. The clamp came loose striking him in the mouth. He went to Welch emg. He hand a broken tooth.
While walking from the Corporate Office to the dumpster a shard of glass fell out of a garbage bag striking the employee on his left lower leg (calf) causing a laceration requiring 25 stiches to close
EE got out of grease truck, walking over to service dozer, stepped in print of dozer track in the dirt. fell down and tore ligaments lose in RT ankle.
