While making a pre-shift a fall was found on the #1 belt 30"" inby spad 42 measuring 20"" wide x 40long x 8""high, after further investgation it was found to a large lip 6' resin bolts was used in complete area.
Electrical power kicked off and electricity to both main fans on all three hoists was lost at 12:04. Power to the mine was restored to the mine at 12:23, #1 fan restored at 12:28, #2 fan at 12:45. Miners began evacuation at 12:10 but restoration of the fans and two hoists allowed miners to return to work.
Roof Fall #3 entry heading. Fall was about 20' X 20' X 5' thick. Fell up to the coal rider.
Roof Fall # 4 Intersection #3R toward #3 entry. Fall was about 20' X 35' X 5' thick. Fell up to the coal rider.
The section foreman on 2nd East Mains discovered a fall in the #5 entry between crosscut #4 and #5. The fall was approximately 10 feet wide, 25 feet long and 10 feet high.
Upon daily inspection of the elevator, it was noticed that one wheel of the guide roller assembly was damaged. The elevator was immediately taken out of service and both agencies were notified. Federal Kemper Rep. stated that the reason for the damaged wheel was a result of being ""too tight."" Repairs were made and elevator was put back in service at 1:10 pm.
The mine encountered a roof fall on the 1st MS, entry #3, S.S. 00+80. The fall measured 30'L X 19'W X 8' thick. The area was developed on 5/10/2007. The fall did not affect ventilation or passage of men, no personnal injuries or equipment damage. The fall was supported with 8"" point anchor bolts. This fall will not be cleaned up.
A roof fall was discovered in the 2 North 3 Mains, 53 crosscut #6 entry. The fall measures approximately 12' long, 5' wide & 10' thick. The area is supported with 5' fully grouted resin bolts, 12' cable bolts and cribbing.
An unplanned roof fall occurred in the #2 gas mine in the main return at spad #156 that measured 32' long x 18' wide x 16' high. No injuries were reported. The area was developed on 8/15/07.
The rib over the inby overcast wall located along 6 North belt at 71 block deteriorated and fell out breaching the separation of the air-courses. Fallen material measured 5 ft by 4 ft by 14 ft. No injuries to any employees.
7 mains 3,6 entry, between 9 & 10 block, mmu 002 16ft wide 2 25ft. long x 10ft high. Suuport 6ft 3/4 inch resin bolts, pans, brow straps, screen. Fall dangeged off, additional roof support installed aroundn fall. clean up plan developed and posted at fall site.
A roof fall occurred in the No. 1 Entry of the 7th sub panel north off of the 8th panel north at spad 6+10 and is approx 7' high X 19' wide X 150' long. There were no injuies, witnesses or disruption to ventilation.
Our examiners found a roof fall in the return of the 5 West worked out area. The fall was located at crosscut 69 between entries 1 and 2. The fall was partially located in the examiners' route of travel.
Roof fall in #3 entry 400' inby the number 2 belt drive. The fall was 30' long, 17' wide, and 5' high at the highest point.
During preheat of Kiln 3 the burner flame extinguished. This went undiscovered by the CRO while the burner continued to send natural gas into kiln. After 3-4 minutes the gas found and ignition source and ignited causeing an explosion causing extensive damage to the heat exchanger and cooler.
An unintentional roof fall occurred on 2 Unit #8 Entry Spad no. outby 1 break 10095. Length approx. 40', width 20', height 4' - 6'. Support being used 4' resin grouted bolt. No injuries occurred.
A roof fall was discovered in the #2 Bleeder Entry at survey station No. 2256. The fall measured approximately 20' L X 20' W X 6' thick and fell in the intersection. The mine was idle during the week of 6/30/08. The fall occurred approximately 550' inby the active section.
There was a roof fall discovered measuring approximately 80'L x 20'W x 10'H between breaks 5 and 6 in the #7 Entry on 2 South Section.
Fall occurred approx 70 ft outby spad #457 in #7 entry. Approximate size of roof fall is 60 ft long, 18 ft wide, 5 ft thick.
16 W section unintended inundation of methane occurred in the face of #3 entry. Horizontal de-gas hole BH-3A Leg 1.
While doing an inspection at #6 shaft, shaft hands discovered that the service brake was not adequately functioning. The brake shoes and caliper were changed and hoist was back into service @ 230 pm, This issue was discovered in a regular inspection.
A new operator was being trained on the 4Shaft escape hoist. The starter box had drawn mositure the hoist could not be operated properly. An electrician was called for and the hoist was operating at 11:35AM.
A 20' x 60' x 4' to 5' thick roof fall, pulling roof bolts, occurred at breaks 85, 86 and 87 at the No. 1 and No. 2 entries of the South Mains.
Accident was reported at approximately 2:00PM. by the fireboss making the area 8P bleeder system. The intersection at #38Brk 8N had fallen from #1to#2 approximately 40'lX15'hx20w. This area is 8290' from the working section of the lw tail, and was mined 06/06/08.
Employee was making pit for the highwall miner. While in the process of retrieving a bucket of dirt he cut an 8"" gas line. He immediately shut off the excavator and shut down the truck waiting to be loaded.
Received a call of a fire in our South Hallsville Mine area. Fire Department responded along with the Harrison County Forestery Department, fire burned approximately 100 acres of pasture and pine trees. Fire was started by an assumed lightning storm that had passed.
A roof fall occurred in the Main West entry #3 at 33 x-cut. The fall measured 20' long 20 feet wide & 8 feet high. It did not affect ventilation, travel or cause injury. It will not be cleaned.
Length 20' x width 18' x thickness 9'. Last open #6 entry (x-cut 18) #1 unit.
A 20'x 20'x 4' to 5' thick roof fall, pulling roof bolts, occurred at the intersection of break 82 in No. 3 entry of the South Mains.
A fire started inside the gear drive for kiln #2. It was put out with CO2 fire extinguishers but continued to flare up. Fire was out at 1:40 pm but flared up at times after that. MSHA was notified via phone. An MSHA inspector arrived onsite between 3:30-4:00 pm.
A roof fall was identified in the old First Left section in the #4 entry inby spad #11297. The fall was approx. 65'W X 20'L X8' high. The area was supported with 4' resin bolts and supplemental 8' cable bolts. The area was mined approx. 6/21/08. The area has been dangered off and timbers have been set.
Roof Fall, 3 West Panel, #5 entry, x-c 13 intake, area mined around 08/18/07 bolts were 8' tension rebar 3/4"" diameter. area had fault which started showing in 12/2007 and the area was cribbed & dangered off in 12/2007. 6/23/2008 the had additional timbers added around the fall. The escapeway was in the #6 entry. The fall measured 25' x 20' x 12' thick
An unintentional roof fall occurred in the old #3 panel (recovery area) at the 67+20 station in the #6 entry. The fall did not affect ventilation, passege of men or cause personal injury. The fall will not be cleaned up. The fall measured 20 feet x 30 feet x 7 feet high. The area had been bolted with 6 foot fully grouted roof bolts.
A roof fall occurred north f 8x #3 Hdg inby feeder. Fall above anchorage approx. 65' long in #3 and 20' into 3x2. Area was bolted with with 6' super twist fully grouted and 12' cable bolts in the intersection. Face ventilation in #3 was compromised. no men or machines were injured or trapped.

An unintentional roof fall occurred in the #2 entry 60 feet outby spad #717. The fall was approx. 16 feet wide x 18 feet long x 70 inches thick. No persons, equipment nor ventilation was affected. The fall has been inspected by MSHA and roof trusses are being installed as roof support.
An unplanned roof fall occurred in the #2 gas mine between spads 352+358 that measured 100' long x 14' to 16' high x 18' wide. no injuries were reported. The area was developed on 12/17/07.
The mine encountered a roof fall on the Main North, entry #3, S.S. 8+60. The fall measured 18'L X 8'W X 6' thick. The area was developed on 1/10/2001. The fall did not affect ventilation or passage of men, no personnal injuries or equipment damage. The fall was supported with 5' & 12' point anchor bolts. This fall will be cleaned up.
#2 Unit, #6 Entry, X-cut 38. Slip and water was present (was already cribbed when fell). Lenght 25', width 18' and thickness 9'.
Using torches in the shaft. Hot embers fell down the shaft, caught bleeder on fire. Took about 5 minutes to extinquish. Called federal inspectors. Cleared to return to work at 4:00 p.m.
12-M seals, between #2 & #3 seal. Rock fell above roof bolts, area supported & walkway rerouted outby to next X-cut and back # 3 Seal, this area is a weekly examination.
Roof fall measuring 30' long, 16' wide and 4' high. Location #3 entry spad # 7227. No path blockage or injuries.
A fall of the roof occurred on #3 unit supply reach at x cut 18 of the 2nd ne panel. 20' length x 20' width x 8' high. Personell, equipment, ventilation was not involved.
#3 hoist tripped on ""Loading Pocket Fault"". An electrician was called out of the mine to get the hoist reset. This was a nuisance trip, so the fault was cleared @ 1:45 am. There were still two operable hoists at the time of the incident.
5th 48 roadway, xcut 31, #7 entry was already cribbed out before it fell between cribs. Length 20', width 18', thickness 6.5' to 7'.
A roof fall occurred on the no. 1 section in the no. 1 heading. The fall measured 20' wide, 20' long & 6.5' thick. The fall has been breakered off and mining is no longer being done in this area.
#3 hoist tripped on ""watchdog UHM"" fault. Electrician was called out of the mine and the hoist was reset and adjusted and back into service at 12:45 PM. There were still tow operable hoists at the time of the incident.
There is a fall on #3 unit, #9 entry, 10 feet outby the tailpiece. The fall is 107 feet long, 19 feet 3 inches wide and 7 feet high.
A roof fall was discovered in the 2 west #1 mains #1 entry 62 crosscut. The fall measures approx. 25' long, 18' wide & 8' thick.
The mine encountered a roof fall on the MNW, 19+25XC, between entries #4 and #5. The fall measured 20'L X 18'W X 6' thick. The area was developed on 7/27/2002. The fall did not affect ventilation or passage of men, no personnal injuries or equipment damage. The fall was supported with 8"" point anchor bolts. This fall will not be cleaned up.
A roof fall occurred in the return of old workings of x-butt room. No equipment was involved.
A methane ignition occurred in the face area while grading bottom. No injuries were incurred.
Preshift examiner discovered smoke and heat in power center location enclosure during preshift examination.He contacted the personnel working inby and removed them to outby the power center location. He and those same personnel extinguished the fire in the power center using water. The fire was contained within the power center structure.
Roof fall discovered 1100 ft in the #1 entry of #1 section spad #3067 in the return, measuring 90'L x 18'W x 7' thick. The fall was investigated by MSHA & WV OMSH&T.
Production hoist tripped out at 6:20 p.m. The cause of the hoist trip was a Drive 2 Fault. A fuse was replaced and the hoist started again at 7:35 p.m.
While shortening service cage tailropes, east side floor of elevator was damaged when chaired. NO INJURIES.
An uplanned roof fall occurred in the #2 Gas Mine at spad #271 in the left retrun at brk. #14 that measured approx 28' long X 22' wide X 12' high. No injuries were reported. The area was developed on 11/25/07.
1st North west main return #2 entry caved, two x-cuts outby spad #3073. Fall was 40 feet in lengh and 20 feet in width thru x-cut 3-2 and 2-1.
Escape hoist for the alternate escapeway was being tested and stopped operating at approximately 7:00 PM; production was stopped and people pulled back, and was back operating at approximately 7:45 PM. It had a relay that was stuck. Day shift went back over the hoist 06/18/2008 to check it out.
#3 hoist tripped on ""east motor overtemp"" due to heat off of the steam heaters. The area was cooled down and the hoist was reset at 5:40 pm. There were still two operable hoists at time of incident.
Electrical outage due to electrical supplier caused #2 and #5 hoists down. Miners evacuated the mine via #4 hoist until power restored. MSHA notified via phone at 7:40 pm 6/17/08.
An uplanned roof fall occurred in the #2 Gas Mine at spad #393 brk. #28 that measured 60' long X 18' wide X 12'-15' high. This is a continuation of a fall that occurred on 4/23/08. No injuries were reported. Area was developed on 1/21/08.
Production Hoist went down because of Drive 1 Fault, loose connection. Loose connection was fixed and the Hoist began to operate again.
A roof fall has occured in the 1st South Main Entrys at 16+80 in #2 entry.fall measures approximately 50' x 20' x 6' and was caused by an undetected slip in the mine roof.Area was supported by 6' tension rebar roof bolts and was mined in July 2007.There were no injuries and ventilation/passage of men were not affected.
Inby brow area of recently shot overcast fell above anchorage, impeding travel in primary escapeway on #6 Sect.,MMU 006, Spad 24876. NO INJURIES.
A roof fall occured on south F #6x #1Hdg outby feeder. Fall was above anchorage 18' wide, 20' long and 6' high. No men or machines were injured, no ventilation or travel affected.
While pillaring #11 entry, #7 lift, the roof fell in covering up the continuous miner with approx. 6' of rock. Cover of approx. 300'. Area bolted with 6' fully grouted resin bolts & header boards. Sub main sect. (19 belt). #11 heading outby spad #4409.
Fire considered to be arson (vandals). Totally destroyed mill office.

A roof fall occurred on the perimeter entry South G Bleeder between #2 & #3 xcut. Fell above anchorage of 6' fully grouted roof bolts; rib to rib unkown lenght due to other end being inaccessable. No persons or equipment injured or trapped. Travel to #5 & #9 B.E.S. is impeede. Good air flow over fall. Main bleeder regulator same.
A ROOF FALL WAS FOUND IN THE RIGHT RETURN (#8 ENTRY) AT 8:55 AM THIS WAS OFF THE THE #2 BELT. THE FALL WAS APPROXIMATELY 10 FEET WIDE, 10 FEET LONG, AND 4 FEET THICK.
A roof fall occured on the main north belt line at x-cut 28 thru 29 and is approx. 8ft high x 20ft wide x 70ft long.

An unintentional roof fall, approximately 20' wide x 30' long x 7' high, was found at approx. 1:20 pm on an idle shift 6-15-08 in No. 5 entry at crosscut 12. The roof was supported with 6' point anchor torque tension roof bolts.
#2 hoist down (electrical switch) no danger to employees. MSHA notified via phone at 1:07pm 6/14/2008.
Methane bleeder encountered; methane ignition aledged to occur. Extinguished quickly; no injury or damage. No citations issued. Note: this was phoned in to the 800# for MSHA.
Roof fall at 3 crosscuts outby Spad 151 between #5 and #6 entries. The fall is approximately 1800 ft underground and 1500 ft from the face. Fall was 16' high, 18' wide and 20' long. The mine was idle. Employees were doing utility work. Fall will not be cleaned up. Fall will be dangered off and supported as per roof control plan.
On Saturday, June 14th at approx. 2:30p.m., a roof fall was identified on the old Mains Section in the #4 entry intersection near spad #364. The area measured 20'L X 20'W X 8'H. The area was supported with 5' torque tension bolts and supplemental 8' and 10' cable bolts. The area was mined approx. 6/12/08. The area has been dangered off and timbers have been set.
Our examners found a roof fall during their weekly examination of the 5 West Mains. The fall was located in the return of the 5 West at crosscut 40, entry 2 to 3. The fall was located in the examiners normal route of travel.
A roof fall was found at approx. 12:10 pm on 6/13/08 by examiner doing weekly exam. fall was at cross cut 53 entry 5 of main west. Fall is 20"" long x 20' wide and at least 10' high. Area had been supported with 8' full resin pins. No injury or property damage resulted. This fall will be cleaned up at a future date.
A roof fall occurred on the South I section MMU002-0 in #4 Hdg #3 Xcut 4 way intersection, 3 Xcuts outby the feeder. 6' fully grouted roof bolts + 4 12' tension cable bolts. No one or equipment injured or trapped. No ventilation or travel was affected.
Unplanned roof fall occurred on grassey creek mains approx.75ft inby survey station.#22166 in the #3 entry between crosscuts #133 and #134. The fall measured approx. 20ft wide x 50ft long x 12ft high.No one was injured,no equipment was caught and ventilation was impaired.Roof fall wont be cleaned up and the area has been timbered off.
While traveling airways, found a roof fall about 4' thick, 25' wide and 40' long, 1 break inby spad #277. Looks like there was a hill seam there and water has cut it loose.
An unintentional roof fall occurred on 3 east mains section in the #4 entry, brk. 127 at S.S. #25282. The fall measured approx. 20ft wide by 20ft long by 10ft high. No one was injured and no equipment was involded and ventilation was not impaired. The fall was timbered off and it will be cleaned up.
Roof fall, about 25 feet long, 15 feet wide & 6' high above anchorage.

An Unintentional Roof fall occured in the 12 H section at 50 x-cut SS# 124+55 between #1 and #3 entry. The fall measured approx, 130 feet long and 16ft high. No injury or equipment damage.
16 W section unintended inundation of methane occurred in the face of #3 entry.
Fall was discovered at 2A belt, break #25 entry #2 about 11:00 A.M.. Fall was 20' X 70' X 6' high. Entrances to the fall were breakered off, and area will not be rehabilitated. Inspector investigated the fall.
06-12-08, @8:05 A.M. a fall was discovered on #2A belt at break 21, in #1 entry, in the return. The fall measured 20' X 140' X 6' high. All entrances to the fall area was breakered off. The area will not be rehabilitated. Inspector inspected the area.
An unplanned roof fall in the active workings occurred between the can cribs that impeded travel from the longwall face into the tailgate entry.
An unintentional roof fall, approx 20' wide x 20' long x 5' thick, was found at approx 7:40am on 6/12/08 in No. 1 Belt Entry at crosscut No. 9. The fall was a continuation of an old fall in this area. The roof was supported with 4' conventional roof bolts.
A roof fall occurred in the 2 south 2 mains, #8 entry, between #17 and 18 crosscuts. The fall measures approx. 30' feet long, 17 feet wide, and 7 feet thick. The area is supported with 5 oot fully grouted resin bolts.
6 Breaks from Main face in #4 portal entry, fall of approx 12"" to 19"" of loose unconsolidated roof. 2 roof bolts turn out (conventional bolts 36""). Fall approx 21' wide by 15' long. Approx 12,000' outby face area in the intake escapeway, impeding traval. (At spad 38 + 60')
Safety switch enalted not allowing elevator to operate properly
A roof fall was found at 2:55 am on South Mains ""B"" beltline at 7 x-cut. The belts went down around 2am and upon further investigation after trouble shooting the belt was walked. Cause of this roof fall was deterioration of existing roof. Area bolted with 8' bolts and resin with supplemental supports installed. Size of fall is approx. 25' long by 50 to 60' wide by 10' high.
A roof fall was found during the weekly air course examination. The fall was located at 12+90 x-cut #20 of the #5 entry of Zone P1. The area was developed 1-15-08. The fall was 90'L x 18'W x 7.5'H. The area was originally supported with 5' fully grouted resin bolts. There were no injuries involved, air flow was not disturbed and the passage of mean was not affected.
A fall of roof occurred at the intersection, in # 3 entry, cross cut 23 in the 1st East. The fall was approximately 30 feet in length x 30 feet in width x 11 feet thick. This area was developed in September 2007 and had been supproted with 6 foot fully grouted roof bolts.
An off site power outage interrupted power to our man hoists.
#3 Unit, x-cut 3, #2 entry. Length 25', width 18', thickness 8'.
A roof fall ocurred on the # 4 main line belt, the fall estimated to measured 20 ft wide 20 ft and 7 ft high No injury occurred at this fall
Unplanned roof fall was discovered 3600' outby #001 section in #1 entry 2 breaks outby spad # 2740. The area will not be rehabed. The size was 80' L x 18' W x 8' H. It was bolted with 6' torque tension bolts.
Unintentional roof fall discovered on above date. Location of fall is No. 2 section, 6 heading, 46 to 47 block, station 5260, Did not block escapeway, no injuries, or property damage. Fall measured 18'W X 50'LX approx. 10'H. Previously supported with 6' full grout resin bolts. Cover approximately 225'. Mined on or about 1/27/07.
Pit was dry and maintained. Miner stated that he was bent over looking into his lunchbox and not paying attention where he was going and veered off the road into the pit. Truck dropped approximately 5ft. coming to rest on its left side, damaging the walkway, handrails. and door.
Inundation of pit/quarry and operations. There are no injuries.
On 06/09/2008 an unintentional fall of roof occurred on the 3rd Main West at crosscut #166, in the #4 belt entry. The fall is approximately 35' long x 35' wide x 20' high. The area was bolted with 10' point anchor roof bolts. The fall will be cleaned and arched.
An unplanned roof fall occurred in the #2 gas mine 1 blk outby spad #721 in the #7 entry that measured 16'L x 12'W x 8'H. No injuries were reported and the area was developed on 4/22/08
Roof Fall on 3 South 2nd Rooms outby working section, #4 enry x-c 6. Area mined in 5-5-08 and showed signs of fault bolted with 8' tension bolts then 16' cable bolts. 5-26-08 area was cribbed off and escapeway rerouted prior to fall on 6-9-08. The fall was 20' Long x 70' wide x 8-12' thick.
Occurred in #2 entry of the 7th panel North at spad 6+26 x-cut 21 in the return. Approx. 8' high x 19' wide x 57' long.
16 W section, unintended inundation of metahne occurred in the face of #3 entry.Horizontal de-gas hole BH-3A.
North 3, 39 to 40 block, 6 entry MMU 004-0. 18ft wide x 15ft long x 7ft high. Support: 6ft 3/4 resin blots, brow straps, pans, straps, screen, jacks and beams. Fall danagered off, additional roof support installed around fall. Fall clean up plan developed and posted at fall.
At 6:55 pm, #2 hoist kicked. The hoist was reset but it continued to kick on uhm error. Electrician trouble shot the hoist and changed out a card in the uhm. There were still two hoists avaiable for use (#3 and #6).
In the 3 I Section there was a roof fall in the #2 entry at the 4 way intersection, spad # 1+31.41. The approximate area of the fall measured 18 feet in length by 18 feet in width. The height of the fall is approximately 10 to 12 feet above the roof line.
This mine experienced a foof fall in #8 entry at the +333 cross cut.
A roof fall occurred in the #3 set up entry of the 12 H section (MMU # 061-0) at approximately spad # 9+33 for a distance of approximately 50 feet. The width of the fall is approximately 15.5 feet and the height is approximately 12 to 15 foot high.
Inundation of the pit and production operations. There are no injuries.
At 11:55pm the mine lost power and had we received over 2 1/2 inches of rain in about 30 min. The diesel pump was pumping, but could not keep up, at around 12:58am water started running into the return portal down to XC 6 into travel belt, and Intake all at xc 6. MSHA was called at 1:06am and a Verbal K-order was issued.
Unintentional roof fall was found by pre-shift examiner in #6 hdg at #53 x-cut (At spad #3178). Fall was 20' Wide X 20' Long and 12' high.
Our examiners found a roof fall during their weekly examination of the 5 West panel. The fall was located in the 5 West return at crosscut 63 in entry #3. The fall was located adjacent to the examiners route of travel.
Our examiners found a roof fall during their weekly examination of the 4 Left 5 West panel. The fall was located in #2 entry at crosscut 22 to 33. The fall was locataed adjacent to the examiners route of travel.
Roof fall occured in #5 entry in the top seam, 3 breaks inby temporary section. Area will be timbered off, bolted, set cribs and clean-up the fall and bolt on cycle.
WE HAD MINED UNDER LOW COVER AND HAD ENCOUNTERED ADVERSE ROOF CONDITIONS. ROPE BOLTS HAD BEEN INSTALLED TO CONTROL THE ROOF WITH OUT SUCCESS.
A continuation of a roof fall above bolt anchor occurred from the previously reported roof fall on 6/05/08. The fall extended from XC 107+66 to XC 108.
WE HAD A ROOF FALL OUT BY THE FEEDER AND WHILE DOING THE INVESTIGATION, WE FOUND ANOTHER FALL 6 BREAKS OUT BY THE FIRST. BOTH FALLS ARE IN LOW COVER AREAS.
Roof fell #2 Entry. Fall was about 20' X 35' X 4' thick. Fell up to the coal rider.
A roof fall occurred in a 4 way intersection #2 and 2 x 3 @ 4 x on south I section. 17' wide x 60' long x 9' high. 2 xcuts outby the loading point. 6' fully grouted bolts with 4 - 12' cable bolts in the intersection. No persons or equipment damaged or trapped. No ventilation or escapeways were affected.
An unplanned roof fall occurred in the #130 mine on the #7 belt, #7 brk in the #6 entry. This is a continuation of a fall that occurred on 5-30-08. No injuries were reported. The fall measured approximately 30' x 20' x 8'.
The governor cable was causing the safety switch to trip on the top of the car causing the elevator to malfunction. Tightened the spring on the car and has taken care of the problem.
Operator saw smoke and pulled over. The operator then saw smoke and fire in the engine compartment when he raised the hood. He then retrieved his fire extinguisher in an attempt to put out the fire, but to no avail.
An unintentional roof fall occurred at the Galatia Mine North Portal at crosscut 13 on the Flannigan travelway 1783' West. The fall was approximately 20' long, 18' wide & an unknown thickness. The fall did not affect ventilation. The fall will not be cleaned up.
A roof fall above bolt anchor occurred in the #1 entry of 22 Right at XC 107+66.
Elevator which provides transportation of men went down approximately 6:30 PM on 6/5/08 due to electrical storm and was not put back into service until 9:30 AM on 6/6/08.
Roof fall in return 500' from surface was found when traveling return with msha EE on e01 inspector.
Excessive rainfall over the past several days has caused the Kickapoo Creek to exceed its banks allowing water to collect in the pit such that normal operations have been stopped. All equipment and personnel had been removed from the pit and were not affected by the water.
An unplanned roof fall occured in the #4 entry near spad #1837 fall measured 20' wide by 80' long by 6' high no persons or equipment was involved ventilation was not affected roof was supported with 5' 3/4"" fully grouted torque tension bolts. Fall will be rehabilitated.
North 8 belt, 6 entry 11 block, MMU 004, 4-way intersection. 17ft wide x 20ft long x 7ft high. Suuport: 6ft 3/4 resin bolts, 12ft cable bolts, pans, brow straps, screen. Fall danagered off, additional roof support installed around fall. Fall clean up plan posted.
During normal mine travel a roof fall was discovered at spad #382. The fall measured app. 19' wide X 25' long X 5' high. 4 foot fully grouted bolts were used during mining on 4/16/07. The fall is in the intake app. 5000' from the face. The mine is non-producing. All sides of the fall have been timbered off. Will not be cleaned.
A roof fall was found in the out by area of South Mains, Brk.93 entry #4, the fall was approx. 7ft high, 30 ft. long, 30ft. wide, the area has been cribbed and dangered off.
An unintentional fall occurred in the #12 entry of the 2nd Main West at the 21+00' 3 way intersection. The area was 30' X 18' X 11'.
An unintentional fall occurred in the #3 entry of the 2nd Main West at the 23+80' intersection. The area was 25' X 25' X 9'.
A roof fall occurred on the Mains Section at break #59 near spad #346 in the intersection of #6 entry. The fall was approx. 25'L X 40'W X 10'H. The area was supported with 4' resin supplemental supports were added including 5' torque tension and 8' cable bolts. The area was mined approx. 5/31/08. The area has been dangered off and timbers have been set.
Slipen sided area spad 47=40 #11 main north main fall 100' x 4-6 x6' 1200 ft outby unit #1.
The tie lines supplying electricity tripped interrupting power to 2 of our 3 man hoists.
An unintentional fall occurred in the #11 entry of the main North at the 40+40' intersection. The area was 30' X 25' X 12'.
The #1 elevator was out of service for a period of time exceeding 30 minutes. All government agencies were notified according to law. No individuals were on the elevator at the time. The retiring cam cable was found broken and repaired. The elevator was tested and placed back into service @ 9:00 AM.
A mechanic found a roof fall in the intake of the 5 West in entry #6 at crosscut 73. The fall was located in the primary escapeway for #2 unit. The section was evacuated after the fall was found.
Two miners were shotcreteing the alimak raise working off of the alimak raise climber when a connecting rod to a piston broke on the lower drive motor. Alimak raise and raise climber is being developed and used as our alternate means of escape out of the mine.
Excessive rainfall over the past two days has allowed enough water to collect in the pit such that normal operations in the pit have been stopped. No personnel or equipment were affected as result of this excessive water in the pit.
Roof fall 2 brks inby spad no. 21054 002 section # 4 Belt. Clean up plan posted. Ventilation slightly affected due to crushed out intake stopping. No injuries. Notified MSHA and state.

A roof fall was discovered in the No 1 entry of the No 3 west marker section about 4000' outby the developing MMU 003-0. The fall measured approximately 30' long x 15' wide x 6' high and was located in the intersection 1 break outby survey station No 6169. There were no injuries or equipment involved.
On 6/3/2008, the entire plant and mine lost electrical power from the supplier. The power outage caused loss of power to the vertical hoist at the mine, however the back diesel power kicked on immediately, and the hoist was operable again. The mine was evacuated at that time. Full power has been restored
A fall of the mine roof was discovered on 6/3/08 at approx 11:45 am. The size of the fall is 80 ft long x 19 ft wide x 10-15 ft high. There were no injuries and no airways blocked.
Methane bleeder encountered. Methane ignition allegedly occurred. Self extinguishing, no injury, no damage, no citations issued.

A rock outburst in the 3700/244 stope caused the withdrawl of miners from the work area following one miner be hit by rock from the outburst while rock bolting at the face.
Roof fall, 4 West at 6 +30 2 room from #1 entry to 2nd X cut in room above the bolt ankorage. Area has been cribbed off and abandoned.
The elevator malfunction occurred with the elevator positioning switch, No injuries resulted doing this occurrence.
An unintentional fall occurred between the #4 And #5 entry of the 2nd Main West at the 10+50'. The area was 50' X 20' X 8'. Fall area was bolted with 6' Tension Rebar bolts.
An unplanned roof fall occurred in the #2 gas mine inby spad #613, #2 section, #11 brk in the left return. The fall measured approx. 35' long x 14' wide x 8-10' high. No injures were reported. The area was developed on 4/11/08.
A roof fall above the anchorage zone of the bolts occurred in the No. 2 face of the 001 section. The fall was approximately 20' from the unbolted face and measured about 20' wide x 15' long x 4' high in the bolted area. No injuries or damage to equipment resulted from this event.
Fatality due to roof fall at the face of a working section. **The fall pinned the victim between the roof fall and the coal rib. Retreat mining was taking place in the 003-0 section at the time. Fall was approximately 30 feet wide, 20 feet long and ranging from 4 inches to 4 feet thick.**
Employee was involved in a fatal rail haulage accident. The accident investigation is on-going. EE-fatally injured while transporting two flats of supplies along the 3-west track haulage. Other ee discovered the victim laying between the clearance side of the track and the mine rib at No. 129. EE was unresponsive at this time. EMT arrived-unable revive the victim.
A tree split & fell onto the victim, pinning him between two trees.
Employee was installing the second roof bolt on the right when a large rock fell from the roof (10' x 5' x 8""-10"") pinning him to the roof bolter. An MSHA investigation is ongoing.
Ripping frozen ground on a bulldozer, while wearing several kidney belts for support.
While repairing the car body of the radial stacker, the I/E was maneuvering a new section of steel box into place, which was balancing on the radial frame I beam. While re-rigging, the the load shifted causing the I/E end to rise quickly, causing his hand to become caught in a pinch point. The employee suffered the amputation of his left pinky from incident.
While accompanied by experienced miner, the injured employee was shutting off the water control valve to shutdown the hydrator process. A pipe thread failure on the 3 way valve released lime slurry onto upper body and face.
injured was installing a roof jack on the corner of the #1 entry last open crosscut. a shuttle car came into the entry. the operator spoke to injured and the proceed into the entry. the rear of the canopy caught injured's left ring finger between the canopy and roof jack resulting in crushing injuries to the 1st joint. amputation at the joint was later required.
The employee had wrapped a thin cable around the brake handle on a piece of equipment and was going to pull the cable but, while he was trying to hang onto the cable it caught on something and pulled tight and caught his left hand and broke his middle finger. The nature and location of the injury caused the employee's finger to be amputated by the doctor.
EE was attempting to align a horizontal pin and when he pulled the relay bar, his middle finger on his left hand was smashed between the horizonal pin and the base lifter.
Individual was standing a beam up from the ground so it could be moved by an Industrial Powered Vehicle, when he felt a burning sensation in his left groin. Was not diagnosed as a hernia until 7/2/08.
Repairing fueling system of truck, he put hand into auger.
He was operating the longwall shearer when a piece of rock fell from between the #12 shield, struck the tailgate drum, flew from the tailgate drum and struck him on the left hand, causing contusions to the hand and amputation of the tip of the small finger.-per call-loss of finger to back end of fingernail-per call W. Dennison.
The employee was on a platfrom 9 ft high helping another employee when he said he ""forgot where he was at,"" stepped backwards and fell to the lower deck. As he hit the ground all of his weight went into his right leg breaking it in 2 places and crushing his heel bone. As he fell he injured his left bicep with 3 scratches and bruising.
Employee felt something pull in his right shoulder while he was bending a 1/8 inch thick piece of metal with a set of channel lock pliers. He previously had surgery on the same shoulder for a non work related injury.
Injured and co-worker were rehanging line curtain in #3 entry when injured bent down to pick up the curtain, a piece of rock measuring 3 1/2' x 5' x 2"" fell from between the roof bolts striking injured in the back between the shoulder blades causing hairline fractures of 3 vertebrae. Injured was treated and released from the hospital.
Injured was a passenger on a johnson rubber tire vehicle that was in the process of parking next to a power center when his right knee struck an electrical plug. This resulted in a laceration which required medical treatment. He was given a full release from the ER physician to return to work on 7/1/08. He in return visited his family doctor and was removed from work.
Employee had finished his shift & was ready to leave site from parking lot. He remembers leaving his overshirt in work area & went back to get it. It had rained much of the day & the area he was walking through was wet. He slipped in a wet spot, caught himself with his left arm/hand/wrist. The result was he broke the tip of a bone in his wrist.
EE was walking up beside the continuous miner as they were setting bits and got a piece of metal in his eye. Started missing work 7/1/08
Getting down off the back of a carbon truck she twisted her left knee.
The employee was decending the ladder from the operators station, using 3 points of contact. The last few steps are wire rope constructed and the wire rope failed. The employee caught himself with his hand, thus pulling at his elbow and shoulder in supporting his weight, until he was able to stablize himself.
Employee bent over to pick up a piece of board that he did not realize was stuck to the ground. When pulling up on the board strained his back.
Employee sprained ankle while walking from Port-o-potty to truck. Walking on even dry surface - road base.
Employee was struck on the side of his ankle by a overhanging brow while bolting. Cause of this injury was safety awareness. Compliance with rules/regs was not a factor, protective equipment not a factor, mining equip/system not a factor, miner proficiency a factor.
Employee reported that he was climbing out of skid steer when his right leg slipped and he hyper extended his left leg. Amendment: Claim was denied by Workmans Comp & is being disputed by company.
The employee was giving maintenance to the conveyor; he was standing in the hallway holding the interior part of the handrail of the conveyor. He got dizzy, falling on hallway, hitting his right leg and affecting the muscle of the thigh.
The injured was operating the radio remote continuous miner when he was caught between the cable handler and the coal rib resulting in crushing injuries to his left leg and thigh.
EMPLOYEE WAS INSTALLING GROUND SUPPORT (ROCK BOLTS). AT TIME OF ACCIDENT WAS DRILLING A HOLE INTO ROCK IN WHICH TO INSTALL A BOLT. A ROCK FROM THE BACK AND IN FRONT OF THE DRILL FELL ONTO A MUCK PILE AND FLIPPED OVER LANDING ON THE EMPLOYEES RIGHT FOOT.
Employee was driving (777) rock truck being loaded with 992G. Another worker was operating D11 helping 992G & hit 777 truck with D11 blade.
Employee was attempting to move a large rock from parking area when he lost his grip letting rock fall on ankle. Rock was muddy and wet. Employee was the grease truck operator and his hands were greasy and slick.
Employee was stretch wrapping a pallet of material and did not pay attention to where he had stationed/placed the portable pallet jack. Employee tripped over the pallet.
The man was walking up the Entry when he struck his back against a metal strap.
Injured employee was standing on the back of a golf cart flagging some bad top conditions and fell off the cart to the mine floor. He received a fracture to his right wrist.
Employee was placing roller in between frame and belt. Roller hit belt and bounced back, pinching employee's finger in between roller and frame.
Employee states that when he stepped off a 1' access platform, he felt pain in his knee.
Working on brattice and a block fell on right hand and fractured pinky finger.
EE stated that while operating a ramcar on the section, he dipped down in a hole & then bounced up throwing him against the roof of the canopy. The impact caused him to hit his head against the canopy. He was semi-unconscious immediately afterwards & then responsive. Taken to the hospital, treated & released. On 7/28/08 he went to a chiropractor, & now has begun missing work.
Injured party stepped up off the ground to the 1st step on the road grader when his foot slipped, striking his leg against the step.
Employee was exiting truck, stepped in hole and pulled ligaments in ankle.
The injured employee was helping stack monorail when his incident occurred. When the employee turned to stack a piece of monorail, he felt a pain in his lower back and right hip.
EE stated she slipped going down wooden steps & fell, landing on her rt arm & buttocks. She alleged pain in her back & neck area. After being examined at the local hospital ER, she was released to return to work. In interview that day & afterward, she stated she was ok. She began missing work on 4/3/09 for unknown reason at the time. Mangagement feels this is not reportable.
Employee was cutting a bolt off the frame of the excavator when a piece of hot metal went into his ear.
Employee struck top with hard hat; dirt and rock fell into employee's left eye.
Employee stated that he was walking off the longwall face and twisted his knee around the stage loader area. The bottom in the area in question was dry with tight clearance around the stageloader.
Driver of water truck was going down an incline when the clutch disk disintigrated causing a small piece of metal to hit him on the lower right leg between the knee and ankle. This required an x-ray and five stitches.
Operating Getman tractor backing the machine. When attempting to repostion body inside compartment(turning to improve vision)placed his right hand onto a guard for the stop block. His finger were wrapped around the edge of the guard. When the machine articulated together, his right little finger was caught by the stop block, resulting in a fracture to the 1st joint of the finger.
Employee was performing maintenance work on a continuous miner. He was changing out a cutter motor. He was using a stone bar to pry the motor into place. The bar slipped out and he fell backwards. He tried to catch himself with his left arm. He sprained his left arm, wrist and shoulder. Last day worked was 6/20/08.
Employee was walking on 10 Rt. section at #16 crosscut between #4 & #3 entry, slipped and fell. He tried to catch himself from the fall and injured his right knee. A MRI was performed and it was determined that a tendon was torn from the right knee cap. Surgery was performed to repair the tendon. Employee started missing work on 6/26/08.
It is beleived that employee had a seizure, fell and struck the back of his head on 3"" gravel. Doctors excuse ran from 6/25/08 thru 6/27/08. He was not allowed to return to work until 7/1/08 pending a drug test and results by orders of company official.
Injured was walking beside railroad cars getting them ready to take to load out and stepped on ballast stone or a piece of coke and twisted left ankle.
Jarred back while stepping out of haul truck.
EE was moving a mantrip from the main line to put on charge at the charging hole. He was facing the wrong way and had his right leg hanging outside the mantrip when he rounded a corner en route to charging hole where his right leg got caught between the frame of the mantrip and the rib causing injury.
Employee was helping with a water pump, he slipped on some rocks and fell hitting his right knee.
Employee's vest got caught on the gate going down into the house of the #71 shovel. Employee turned to release his vest and felt pain in his right knee.
Employee was walking in a low top area; he ""ALLEGEDLY"" turned his back when he raised up in a higher top area. Mine management was not aware of the injury until he was scheduled to return to work after the vacation period on 7/8/08.
While using his feet to push a tire, EE hurt his lower back. He began missing work 10/2/08 from an injury that occurred on 6/25/08 that was diagnosed as degenerative disc disease.
Employee was reaching up with his right hand opening a valve when he felt a pain in his right eye. The pain was off and on all shift. When examined the doctor found a small object in his eye.
Injured was talking with another employee who was sitting on a mantrip. Injured leaned over onto the fire extinguisher handle. The fire extinguisher discharged causing injured to inhale the chemical resulting in chemical burn to lungs.
Truck was traveling across an area that had recently been wet with the water truck. The truck began to slide. The driver turned the truck to the right. The truck hit a dry area of the road and overturned. The truck's cat walk, hand rail, mirror, door & window were damaged.
The employee was struck by a piece of rock that measured 4' x 2' x 6"". The employee was a greaser and was lubricating on the #3 section when he was struck.
EE was stacking bags on pallet and over-extended right shoulder and neck, causing muscle strain.
While operating bolter in #4 heading of 1st right section, employee was drilling a hole in roof to install roof bolt when he applied too much thrust with less rotation. This caused the drill steel to bend striking his right hand. He was holding onto steel with hand at this time. He had no deformity to arm or hand but did have swelling.
Employee waw taking antifeeze over to the dozer area. While standing near the dozer station area, he took a step to the right and rolled his right ankle. He was taken in to be checked and taken off work for approx. 4 wks. Currently ONGOING.
This electrician was welding on the boom of a miner when he sustained a flash burn to his eyes. Went to doctor on 6-25-08. This was his second day at this mine.
Injured was puling charger cable 4/3 cable from break #5 to the electrics. The cable got caught on a crib, pulling him down, straining his groin. Injured was taken to a medical facility. He is being treated for a srained groin muscle. Employees have been contacted to get help.
Injured was walking by the lights for the #23 Drail on 8Q when the light exploded. The glass made several puncture wounds to the right ear area. Injured was taken to medical facility. Investigating why the light exploded.
While loading 8"" HDPE, employee was thrown to the ground when he pushed the pipe while it was on the forks and it became out of balance.
Employee was removing a radiator hose from a truck. He slipped and pulled the hose off. Hot antifreeze burned his left arm and hand, second degree burn.
Miner was barring down at drift face. A rock was brought down that bounced/rolled over employees foot. Employee was not wearing full metatarsal protective boots, just steel toed rubber boots. The impact of the rock created a laceration and broke two bones in the employee's foot.
Employee claims that while carrying his lunch bucket and a bucket of miner bits from the elevator doors to the 6 ton battery station. Employee stated that he felt pain in his back, employee visited the local hospital and was seen by the ER and was removed from work at this time. 6/24/08.
Employee was filling up a bulk tank on service truck, when he started to exit the back of the truck he lost his footing and fell striking his rib cage against the top of the ladder. He was transported to hospital and diagnosed with two broken ribs. This accident was discovered in an internal audit as not being reported.
On 6/24/2008, a piece of draw rock struck the employee's head, neck & back. Immediately following the accident, the employee was treated & released from a local hospital, but has since sought medical treatment. Then, he began taking off work on 5/18/2009 as a result of this accident/injury.
Employee was getting out of his loader when his feet slipped on the mud on the step. He caught himself from falling off loader with his right arm and sprained his right shoulder.
Miner was struck on the foot by a rock that came off the shearer.
Employee was assisting loading belt in the supply yard, when he tripped and fell and strained his back.
Employee was operating a haul truck. The employee struck a bump in the road and experienced pain in his neck and back.
Employee alleges to have felt or experienced low back pain after handling a track tie in #2 Entry, F-16 Longwall Section. Employee has been under a doctor's care for at least the past 2 months for low back pain.
Employee was working at a gate. A bucket truck drove through the gate, the bucket struck the employee on the head knocking her hard hat off. Employee fell to the ground to get out of the way of the truck.
Employee was loading belt roller into scoop bucket and pulled muscle in his back. He came back to work on 6/26/2008
Driver was sweeping out cab of truck, not pay any attention on what he was doing and walked backward off the catwalk of the truck.
EE & his co-worker were working in Evap in the HI3 unit turnaround. They were heating a pipe that needed to roll for flanges to come together. A rose bud and a maul were being used. While hitting the pipe, scale fell from the pipe hitting EE on the top side of the left arm. Medical evaluation revealed a fracture to the left radius which will require surgical repair.
The man was assisting in preparing to install a roof strp. The strap slid and struck the man on the chin.
Employee had removed his glasses for cleaning, while wiping them and glancing around a small piece of rock fell striking him in his left eye. Upon examination, the Dr. took him off of work.
Employee was cleaning floor with a wheelbarrow and pulled his back when the wheelbarrow tipped to the side with material in it and he tried to stop it from upsetting.
Miner was climbing the outside of an inclined square chute, using welded on angle iron as a hand and foot hold. Hand hold broke loose, causing miner to slide approximately 4 ft. Miner's arm contacted a sharp protruding piece of metal, resulting in a severe laceration to the right inner forearm.
EE just sat down on ride to move it inby so pinner could pull up in #5 and one piece of draw rock fell out on him.
6/25/08 EE called to say he was unable to work. He said on 6/23 he was loading the TC sprayer (plaster machine) & liquid shot into face & eyes. I asked about safety glasses. He said he broke them, they were on his head but not on his eyes. His eyes were washed at work. He returned next day though his eyes were a little swollen. Today they're swollen worse & he is going to dr.
Miner operator was cutting in #3 entry, he was 4 rows of roof bolts back from last row of bolts, a piece of rock fell on right rib 93"" long 27"" wide and 3"" thick, hitting him and landing on his right leg. He was transported to Hospital for treatment, exray showed a fracture to his right leg.
He was told to wait until the motor stopped, before he lined the draw heads up. The motorman pulled up 4' and stopped. The motorman told injured that he would pull up again and wait until he stopped before he lined the draw heads up. He then heard injured holler. employees have been contacted about working around moving equipment.
Shuttle car operator bumped head on top.
EE was picking up 6' timbers and putting them in the scoop bucket and turned wrong. Did not miss any work until 7-5-08, making it reportable.
Injured was moving a tool box when he felt pain in his stomach muscles.
EE was removing a motor & bracket from side of airlock. EE had left hand resting on lip of airlock & was using right hand to remove bolts. EE was removing last bolt when bracket swung upwards and caught his left little finger in pinch point. Fractured little finger on tip and laceration requiring stitches.
Employee was lowering a 14"" diameter pipe into place when it became caught. He kicked it with his right foot to free it. The pipe broke free catching the side of his foot in a pinch point causing a laceration. The employee was wearing metatarsals, but it caught just behind steel toe and below metatarsal.
Miner was fueling the plant dozer and the fuel hose was wrapped around his leg. When he finished he dropped the hose, he than lost his balance and fell from the rear platform on the dozer. EE pulled something in his shoulder and is awaiting diagnosis.
Employee was driving the Polaris from the office to the mine site when he took a turn too fast. The vehicle started to tip over so he put his left foot out to try to stop it from tipping over. When he did that the vehicle tipped and pinned his left foot underneath until it came to a stop.
EE was riding on the scoop (He should not have been there), his foot slipped into the point (scoop part that swivels).
EE was putting the belly pan back on the miner when the lifting eye broke off letting the belly pan drop approx. 16"" landing on the employees legs.
Injured was helping to pull slack in new power cable when his right knee popped.
Servicing excavator. After refueling he was in the process of returning the nozzle to the grease truck. The nozzle was not swiveling properly and had leaked some fuel on the tracks. The nozzle gave a twist when he pulled it out and caused him to fall. He landed on right knee & fell to the ground, hitting the back of his head on the tracks. He was unconscious for a few minutes.

Employee with 30 years of mining experience fell from a conveyor structure and landed on his side. He never lost consciousness and complained of pelvic pain and tight breathing. He did not the JLG man-lift with harness to access the elevated surface as trained.
Employee reached over top of scoop while scoop was still energized. When he sat back down in the operator's deck, his body caught the steering lever causing scoop to steer itself under low roof, catching employee's body between roof and the scoop.

Employee was mounting a shovel. His left foot slipped off the ladder causing employee to fall back. Employee held on to the rail with his right hand causing pain to his right side.
Lifting ANFO Bags on to Powder Rig Trailer, employee felt a pain in his back.
EE reported: He and coworkers were preparing sled to begin storing reclaimed belt supplies onto. As he was kneeling in front of the sled, a piece of draw rock measuring 7'x3'x0""-2"" thick fell striking the top of his head, sliding down his back and knocking him to the side. Extent of injury was pain and stiffness to his neck and upper back region.
Employee stated that while pulling 7200V cable on the right section he felt a small pain in his lower back. After finishing the job and going to the 3 right section the pain in his back became more severe and he was unable to finish the shift.
Loader operator slipped while descending equipment ladder, spraining right foot.
Employee was cutting a tree that had a grapevine in the top, when the tree started falling the grapevine redirected it toward the cutter, striking him. EE sustained a dislocated shoulder and a small cut above the eye.
While opening a 10"" knife blade of the bin activator on a 50 ton lime silo, lime sparyed from around the opening and covered EE's head and face. The EE was wearing required PPE. While attempting to wash lime from his head, lime entered his eyes and caused irritationand minor burns.
He was operating shuttlecar, loading behind continuous miner. Miner hit tail of sc. He hit his head in top of canopy. Strained back.
EE was climbing a ladder to tie the ladder off; the ladder shifted and started to fall. A spotter was holding the ladder but could not hold it when the ladder shifted. The ladder and employee fell approximately 12 feet to the ground, with the employee landing on top of the ladder.
Employee slipped off track of dozer while changing fuel filters, jarring lower back.
Attempted to stop lime from spraying from the opening around the 10"" knife blade valve by sealing the opening with tape. The EE was wearing a respirator, but no eye protection. The dry lime struck his repirator and entered behind it. After exiting the building, he rinsed the lime from his head and face with water. Caused irritation and minor burns to eyes and face.
Employee was training another employee which required both of them to be in the cab of the vehicle. The employee stated he was required to sit in an area that had no seat. This plus the movement of the truck caused back sprain. This employee had a similar injury prior on 10/16/06.
ON June 19, 2008, EE was putting bolt in corner of No. 1 entry and corner rock rolled out, off rib, striking EE on back of his head. Rock measured apprx 24"" X 12"" X 12"" thick.
Received a doctors note on the 26th. EE supposedly was loading bolts on bolter, when he hurt his back. Nothing was said to his foreman at time of accident. At end of shift on the 19th, I talked to EE personally, nothing was said to me. He seemed fine.
Employee was exiting his rock truck at the end of his shift. As he was climbing down the ladder his foot slipped causing him to fall approx. 10 feet to the ground. He landed on his left arm. Employee broke his left wrist.
EE reported: He and a coworker were unloading a 30 hp submergible water pump from the rear seat of a 4-man diesel ride. As the pump was raised using a chain and come-a-long, the chain slipped at the hanger, slid sideways catching his finger resulting in a small laceration to the tip of the left little finger.
An 800 amp fused disconnect was turned off and locked out with only power to the line side. The door was opened to prep tools for a job. Employee set the tools on the top of the box when one of the tools rolled off the box. As he tried to grab the tool it fell into the top side of the switch and shorted out. All arch shields were in place.
Employee was using a torch to cut a conveyor frame. When he cut away one portion of the conveyor frame the other portion where he was tied off to began to collapse and slide down a hill. As he tried to jump free, his safety harness pulled him into a conveyor cross member, striking his back and right side. He was taken to the local hospital by ambulance.
Employee was welding @ mill when a welding spark went behind his welding shield hitting his left eye. Piece of weld in eye.
Pulling klinkers, he felt severe pain in shoulder and neck.
Employee had come-a-long hooked to a bracket on each side of tilt cylinder, holding it up to change out pins. Employee was using his finger as a line-up tool. The bracket broke and caught his fingers between the cylinder and hole in clevis.
Injured employee was in the process of installing permanent roof support, he had put together the roof plate and resin rod to be installed, pitched them toward the drill boom, as he was walking toward the drill boom, a piece of draw rock fell from the outside roof bolt toward the rib, striking his right shoulder and right foot, resulting in fracture to his foot.
EE was walking through shop, when his knee gave out and he fell on shop floor. EE now states that he tripped and fell on the shop floor.
Injured while helping to build a brattice; stepped on a nail protruding thru a wood wedge. The nail went thru his boot puncturing his left foot.
Turning water on wash down hose and hose whipped and struck employee in the nose.
EE was trying to loosen a 6inch pipe nipple from a 90 degree elbow with a 42inch pipe wrench, when the wrench slipped it caused EE to fall and twist his right ankle.
Employee bent over to pick up gloves that he had dropped felt pain in lower back as he bent over. He completed shift but his back continued to hurt. Went to dr when shift was complete.
The injured employee stated he was in process of checking an air leak at the main water stand. While walking toward the air valve location, he stepped on a loose rock. When this happened, his foot rolled off the side causing him to twist his knee. The injured continued to work until his physician took him off work on 6/27/2008 due to the MRI showing a torn cartilage.
DID NOT START LOSING TIME UNTIL SURGERY ON 7/22/08. Moving a coupling and felt strain in abdomen. Umblical hernia.
While dumping wet spoil material hauled from off site, truck tipped on its side. Due to fact material was stuck inside dump and driver was on sloped surface.
This accident injured a contractor who was working by himself at our mine site. His was reparing the hammer (rock breaker). Somehow his hand became caught in the shaft by the cylinder. His hand received major abrasions to all four fingers.
Employee was filling d-8 dozer with fuel and EE slipped off the dozer track.
Employee climbed onto platform at shrink wrap machine on the Packing Line to pull jammed plastic off of machine. As he stepped down from the platform to the floor he slipped and struck the floor with his left elbow. Injury sustained was a fractured left elbow.

Employee stepped out of the doorway at the roof entry point and onto a wooden walkway constructed of spaced 2x4s. Two of the boards were broken covered with OSB board. She stepped on the board, which apparently slipped and she twisted her ankle.
EE along with two co-workers were in the process of loosening the winch cable rachet to loosen hard rigging to separate barges. EE & co-worker were pushing on rachet handle to get slack when rachet slipped striking employee. (This was reported to & filed with Dept of Labor Longshoreman because it occurred on the US waterway (river).
EE was helping move a 245LN miner, when he reached for the cable, he put his left hand on the cable standoff when the back of the moner came up and caught his arm between the roof and the frame of the miner fracturing his left arm near his wrist.
EE was installing a 5 foot resin roof bolt with 8""x8"" plate & a metal curtain hanger. In an effort to align the hanger he attached a nylon rope to it, when he applied rotation to the bolt his hand & arm became caught and twisted by the rotating roof bolt resulting in fractures to his left arm, wrist, ring and middle fingers as well as a laceration to the palm of his left hand
A small piece of coal rib fell against right side of roof bolter, leaving a scratch and bruise near rib cage.
Miner was driving Mack water truck between plants when he hit a pot hole in the County road causing strain to the miner's neck.
While working at the tankhouse, the EW Operator experienced muscle cramps, dry mouth and was treated for dehydration.
Associate was climbing out of powder truck, when he fell and injured his back.
While lifting relay switch into position with right hand, employee placed left hand in a pinch point and pinched off the top of pinky finger on left hand.
Employee was filling conveyor drive hydraulic reservior using a 5 gallon bucket. Employee lifted the bucket and turned to his right and foot slipped on oil. The employee reported a sharp pain to his left knee.
EE was lifting a gas can off of a truck and felt something pop in his back.
Employee was climbing a stationary vendor ladder attached to equipment. He was grasping for the next rung and his hand slipped and he fell backwards 4'. His back hit against an adjacent railing.
A mechanic was changing the front tire on a 15 ton Ford dump truck. The mechanic removed the bad tire and leaned the tire against a hand rail. As the mechanic walked away from the tire, it fell and struck the mechanic in the back of the leg. This caused the mechanic to fall backwards, towards the falling tire. The incident resulted in a broken leg.
EE was picking rock off belt line at the clean coal crusher, when he lost his footing and fell.
Employee was walking on a slick surface, slipped and caught himself without falling. Employee then felt pain in the lower abdomen. The condition was diagnosed as an inguinal hernia.
THE MAINTENANCE EMPLOYEE ATTEMPTED TO NOTIFY THE LOADER OPERATOR OF HIS APPROACH TO PERFORM MAINTENANCE ON THE LOADER. THE OPERATOR DID NOT HEAR HIM AND WHILE MAINTENANCE EMPLOYEE WAS CLIMBING UP THE LADDER OF LOADER, THE LOADER OPERATOR PULLED OUT CAUSING HIM TO LOSE HIS GRIP.
Employee was in a manbasket on a forklift painting a warehouse loading dock canopy. When the forklift operator moved the forklift, he misjudged the distance between the manbasket and and a steel girder, and momentarily pinned the employee between the girder and the back of the manbasket.
A coworker picked up a piece of floor decking; a piece of 1"" X 3' to 4' rod fell from the decking to the next floor level striking the empoyee on his left hand.
Employee was cutting weeds on hill beside belt conveyor #11 with hand held gas weed eater. He stepped into a hole twisting and straining his right knee. (Note knee surgery May 1, 2009)
After wiping mirror clean on truck, the employee stepped to the ground. His foot came down on a stone that rolled under his weight, causing a sprain to his left ankle.

Running & lifting (installing) 6"" + 3"" steel lines causing sore back. *Miner was cleared by doctor to return to work with no restrictions on 6-22-08. Miner quit on 6-20-08.
EE was climbing down the ladder of the roof bolter. He had a water cooler in his hand while descending the ladder. He was approx. five feet from the bottom of the ladder, when he lost his footing and could not hold on to the rails of the ladder. He then fell to the ground breaking his left, lower arm.
Employee was installing roof bolts when he lowered the bolter boom on his right toes, causing fracture to two toes.
Employee was working on power box putting in a switch when his finger got caught causing a laceration.
Employee was operating a hauler, it caught on fire, he had to exit the hauler, went down three steps and then jumped to excape the flame spraining his left ankle.
Diesel Man Bus engine solenoid would not pull in enough for the engine to start. The engine cover (hood) was raised to push solenoid in by hand. The vibration of the engine starting caused the hood to fall striking his left hand and right middle finger resulting in a contusion of the left wrist and a Fx distal tuft right 3rd finger/laceration.
EE was bent down nailing a spad to hold down a curtain, when a rock fell from the roof and struck him, knocking him to the ground.
Employee was working in the cool hill 3 intake aircourse. He was machine rock dusting the air course, he was holding the rock dusting hose and walking backwards, he stepped in a hole and felt pain in his lower back.
Working on stoppings, crossing over gob pile, stepped on loose rock, rock went out from under foot and to keep from falling on top of some H beams, EE grabbed a hanging roof bolt and pulled shoulder. Accident 6/14/08 never became reportable until 7/9/08.
EE stated he was in the process of unloading belt structure out of the scoop bucket, he entered the scoop bucket after scoop had been stopped and shut off to get a piece of structure and stepped back out of bucket. When scoop operator let bucket down it hit on his left foot.
Employee fell approximately 3 ft off the take-up dolly frame, while trying to remove a set of 3/4 to come-a-longs.
EE stated that he reached to take the drill steel out of a hole and felt pain in his shoulder. He has not missed any work. He was given a MRI on 09/05/2008 and the results showed that there were no tendon tear identified. Possible tendinopathy. Also fluid was found in the subacromial bursa (bursitis) this is not an injury. Started missing 10/02/2008. Surgery
He was sitting in the shuttle car while it was being loaded. A piece of rock flew from the tail of the miner and struck the man on the chest.

EE was helping make a power move, when he tripped on a power cable and pulled muscle in his right leg.
EE pulled something in his lower back while shoveling mud from under section tail piece onto belt.
The victim was walking along and stepped on a sled runner with one foot and a piece of coal with the other, the coal rolled and he fell forward. His foot was caught between sled and runner. Fractured both bones in lower left leg.
Employee was pulling miner cable out of roadway and fell, landing on his left arm.
Operating Fletcher roof bolter in face area, when attempting to drill the rib bolt hole, he hear the roof pop. He leaned under the machine's canopy attempting to protect himself. A piece of draw rock (5'x4' thick by 18' long) struck the back side of his left calf resulting in a laceration.
EE was standing on a ladder, one foot on the ladder and the other foot in the generator room. In an attempt to pull the breaker, he slipped and fell approximately 5 feet, injuring his back and hip.
Employee bent over to pick up a belt roller, when he stood up to place the roller in the top structure, he felt a pop in his abdomen.

He stepped off a ladder and twisted right ankle when foot contacted ground. Worked for rest of day and next. 6/16/08 went to doctor to have it looked at and doctor had him stay home 2 days. Returned to work 6/18/08.

EE had finished loading a mortor truck when a bar rolled off the loading platform and hit him on the head.
Cutting bolt off of screen with a cutting torch, a piece of metal flew off and hit pant leg, catching pants on fire.
Standing between roof bolter and 2 man buggy in break between #6#7 entry. A ram car came around block in #6 entry going to Continuous miner hitting 2 man buggy and pushing it into the roof bolter, catching his right leg between the two pieces of equipment. Started missing work on 9/12/08.
While the injured was splitting a rock with a chisel. The chisel head broke and a small piece cut his arm requiring 3 stitches.
Employee was walking through a vent curtain. He had his right hand on a crib and a piece of rock dislodged and struck his hand. This resulted in a broken bone in his hand. He was not operating any machinery.
Carrying a part from the storeroom to dumpster the employee was walking on uneven ground a felt a strain in his knee.
Employee was operating a haul truck and was jarred when the shovel dropped the load into the bed of the haul truck.
EE was on the highway to the strip job to get a load of coal and had to turn around, due to a piece of state equipment in the road. He got out of his truck and stepped into a 3-4' ditch, hurting his back. Went to ER for X-rays, they were clear.
Employee stepped onto concrete pad to enter 149 tunnel, when he stepped on a rock and hyperextended his left knee.
The employee was shoveling belt and felt a pain in his lower back. X-ray was negative and the employee returned to work. This employee notified management that additional back problems from the previous incident required him to return to the physician. Additional rest and treatment was prescribed. (Started missing 8/22/08).
EE was stacking bags onto pallets while waiting for the conveyor to be fixed. He felt back pain upon awakening the next morning (about 6:30am 6/13/08) EE reported injury to his supervisor at the start of the shift.
While being assisted by a 2-man ride down the #9 seam slope, the golf cart lost its brake and the front of the golf cart struck the right side of the slope. The 2-man ride stopped and the golf cart turned slightly sideways, slamming into the back of the 2-man ride pinning the employee in the golf cart seat causing a laceration, fractured pelvis and two fractures to the vertebra.
Employee operates a front end loader repeately and digs into hard face mountain. Drives over a rocky road. Injury occurred over time; not a specific occurrence.
The man was drilling a hole in the roof at an angle when the drill steel coupler broke. The drill steel struck the man on the lip.
EE was reassigned to bulk operations on 6/9. He said his knees were sore on 6/12. On 6/16, he was sent back to his normal duties. On 6/23, he stated that his knees were not healing and that he could not perform the climbing that bulk operations require. He was sent for medical attention on 6/23 and became reportable on 6/24.
EE (along with 2 other men) was hanging miner cable. Conditions were muddy and the cable was pulled tight. When EE raised cable over his head, he felt his back ""shift"". Initial diagnosis is slipped disk in lower back.
Employee was stepping up on the ladder of loader, pulled his lower back and hip out. Has pain in lower back and hip socket on right hip.
Employee's shift began at 7:00 p.m. on 6/10/2008. Employee was operating a rubber tire and it was struck by a boulder while cleaning a shovel pit.

EE was removing radiator hose on Cat 992 loader, when hot anitfreeze splashes on his left arm. Burned top of hand and arm.
As employee was exiting from under a 992G endloader, he raised and struck the access ladder with his head.
Taking arm off of water truck and wrench slipped and hit underneath dash causing a cut to hand.
Employee on 001 section, #1 entry; using a 4 lb hammer to dislodge drill steel from chuck on drill pot. He missed the steel, struck left knee with hammer. Completed shift, went to E.R., referred to Orthopedic specalist. Attended appointment 6-17-08, Ortho took off work for a week with follow-up.
Employee was standing next to his dozer. As he turned to talk to a co-worker, he felt a pain in his lower back. He completed his shift and went to the doctor the next day. He was advised by the doctor to take some time off work.
Employee was hauling overburden in an end dump to the truck dump. As the end dump was waiting to back to dump berm, he was struck by the ripper of a dozer as he was backing off the dump. The collision jolted the end dump operator and he stated later that his neck was getting stiff.
Using a front end loader, loading rock. The loader bounced and dirt came out of the AC vent and blew into his eye, causing a scratched eye.
The employee was walking across the section when he slipped on wet and muddy floor. He caught himself on the rib. He felt pain in his back. He worked until 6/27/08 when he left for vacation. He did not return from vacation due to his injury. His first missed day was 7/7/07.

Employee was carrying a capacitor up the shovel stops when he felt a pull in his groin area. He was taken into the ER to be checked out. He was taken off work and referred to a surgeon. Employee had surgery to repair a hernia on 6/13/08.
Employee was on the catwalk located at the c-19 conveyor. He was assisting 2 other employee with the re-spooling of the cable. A section of the conveyor ""dropped down"" the belt went into a ""wave like motion"" causing them to be tossed upward off the belt, and the cables somehow whipped in the air and wrapped around his legs, pulled him upward, then dropped him.
While riding out of mine (end of shift), a piece of roof rock fell out and hit emoloyee above right eyebrow, casuing a laceration.
Employee was roof bolting he was putting in pin steel and pulled wrong lever and boom came down and hit his right hand bruising it.
EE was operating a continuous miner on the 004 mmu section and was turning the right crosscut in the #7 entry. He was positioned on the left side of the entry, approximately 12 ft. inby the miner and a piece of draw rock fell from between bolts and struck him on the head, back, and legs. The rock was approx. 7""thick x 4' wide x 9' long.
Employee was entering the compartment #5 of #4 dust collector. Employee bumped/scraped elbow on the narrow access doorway when entering to do work.

The employee was lifting bags of cement for the blending of material when he twisted his back.
Employee was pushing shot rock to the 1800 excavator when one track of the D 10 bulldozer he was operating climbed on top of a large rock. When backed off the rock the dozer fell quickly causing the employee to hit his head and shoulder against the cab of the dozer.
The support cables on c-19 conveyor became twisted. They were working on re-spooling them when a section in the middle of the conveyor frame ""dropped down"". When the section dropped, they who were standing on the belt were jolted upward off the belt due to the ""wave-like motion"" on the belt. When they came back down on the belt, they sustained injuries.
Operator was mining wall throughout the day, When about 4:20 pm operator was backing out from wall when wall collaspe. Material fell on front end of loader and hit windshield, causing windshield to break and laceration to operator left and right arm. Highwall is about 25 to 30 feet high.
Victim was leaving job at the end of shift, he went around the water truck that had stopped along the haul road and struck another vehicle that was coming onto the job. No injuries in other vehicle. Victim was taken to the hospital and x-rayed : no broken bones just bruises. Missed one days work because of being sore.
Employee was roof bolting when a piece of slate rock fell and hit him on the left hand. Rock was 1' x 2' x 2"" thick.
Employee stepped from scales to deck to clock in and in doing so turned his foot causing pain in his lower back.
Unsecure false roof fell on miner striking him on the head causing a concussion and also causing a knee injury due to hyperextension.

EE was helping to install the gas pipe on the kiln, the gas pipe spun pinning his right index finger between the gas pipe and forklift rack. The right index finger-tip was broken and required surgery.
Employee was climbing a ladder to hand another employee a 55 drum lid. He was approx 3 to 4 rungs up on the ladder when he reached to hand up the lid, the ladder then kicked out, causing him to fall off approx. 8 ft. The ladder was tied off on one side of the ladder.
Was helping load mtls in scoop bucket when slipped and fell.
Employee was taking bolts out of a guard when the guard slipped off falling down striking his right hand and cutting it.
Electrician alleged that he was throwing a leveling chain on the roof bolter when he lost his footing and fell straining his left hand.
EE states he dislodged rock from scalping screen and lost his footing, falling down the chute onto refuse rock. EE suffered contusions on lower back, tailbone and side and also numbness in leg. EE was not tied off at the time of the injury.

A roof bolter operator was in the process of prying down a loose rib at the time of the incident. When he pryed the loose rib down, a piece of rock above the coal fell from the rib, striking him on the right leg and knocked him to the mine floor. Fractured his leg below the knee.
The man was lifting his tool box when it slipped out of his hand and struck him on the left middle finger.
Victim was entering job at the start of shift, another car went around a water truck that had stopped & struck his vehicle coming onto the job. Originally reported as no injuries in this vehicle until 6-13-08. Victim went to the doctor and was told he possibly had whiplash. Missed 4 days work:returned 6-19-08.
On 6/9/08, EE was walking up No. 2 entry to fork lift chagrer and stepped on a rock in road way, twisting right knee. He was referred to a specialist on 6/16/08 and specialist took him off work on 6/17/08.
Employee was giving the contract driller a piece of duplex wire. When he turned to go back with the shot crew he turned his ankle in a crevice that was created from the drill riggers.
EE was running SC and hooked back fender in #7 entry. When EE turned corner his canopy caught the corner, broke off a piece of rib and it came in the deck and hit his head and left leg, causing injury.
Employee was spraying a launder at the #3 ball mill and began to experience pain in his back.

EE was standing along the stage loader that was being installed for the new longwall panel. The stage loader was sitting on crib blocks about 1.5 feet off of the floor. While attempting to hook another piece of the stage loader together, it fell off of the blocks and knocked the EE to the ground. 12 days later the EE went to the Dr.
EE was walking down the entry to use the phone, when he stepped on a lump of coal and twisted his right knee.
Employee was operating dump truck on unlevel terrain, while dumping load, truck began to rock and the truck tipped over. Employee failed to position vehicle on level ground. Employee injured leg and ribs.

The EE was soldering a piece of metal and he pinched himself with the metal. While holding the metal piece, it slipped and pinched the index finger of the left hand. The finger was fractured and required stitches.
While shoveling out a elevator pit employee felt a burning and pain to his left groin. He went to the Doctor and has a Hernia. He was operated on 7/7/08 and will be out of work for about a week.
Employee got truck out of gear on hill to slow truck down. He steered truck into drainage ditch; impact from going into ditch caused water tank to become loose and come into cab on driver.
Climbing ladder to get onto a bag to pump mix, ladder had mix on it. He slipped off the second step from the top, turning his left knee.
Employee states that he pushed on a door and this caused him to sprain wrist. Wrist did not swell up or hurt until he woke up from sleep the following day.
Employee was operating a jeep on the track. The trolley pole came off the trolley wire. He reached out and grabbed the pole and felt something ""pop"" in his left forearm. Last day worked 7/2/08.
EE was working in a new shot making a loading pad. The material was on a different level than the previous shot. In order to move material, he was running the loader bucket into solid rock. On one of the passes he felt pain in stomach. The pain increased until he called the foreman and transported to ER. CAT scan found hernia. Started missing work on 7/16/08, history of hernias.
Employee was greasing equipment, when grease fitting exploded and struck him in left testicle.
EE was lifting pieces of stone onto a pallet when he sustained a sharp apin in his lower back. Dr diagnosis, acute back strain, EE will off work until authorized to RTW by Dr.
Victim was spraying fire proofing plaster on the ribs at the scoop charger station and it got under his safety glasses and into his eyes.
Employee went into cab of locomotive being serviced to get pry bar, and fell in the cab where the floor board had been removed while repair work was being done on the locomotive. Employee continued to work his regular job until his scheduled surgery on 9-10-08.
Employee was carrying jack to roof bolter when he stepped in hole and twisted his lower back. Initially was a medical treatment. When he visited chriopractor, became a lost time 6/9/08 when he started missing work.
EE was filling a hydraulic oil tank on a 992c loader. He climbed down the ladder far enought to see the sight glass, while on the ladder a step broke loose and caused him to fall to the ground.
Injured EE and another EE were lifting electrical cables on to the conveyor belt so that the belt entry could be scooped. Injured EE twisted his back and was injured. The injury resulted in a bulging disk in EE's back.
Section of high wall collapsed, causing a rock to come through the front windshield of front end loader. Employee recieved a laceration to his left thumb. Did not require treatment until 08/14/2008, when a piece of glass was removed.
EE was tightening the top clusters during belt move. He was on the offwalk side and the rib blew out and in the process of getting out of the way he twisted his knee.

EE was performing maintenance on secondary crusher, when he dropped a wrench. He stepped off the platform on the secondary to the conveyor to retrieve the wrench. In the process, he turned his right ankle.
Injured party left work early on 6/16/08 saying his back was hurting. When questioned about his back, he then informed us that he had hurt his back on 6/5/08 but did not tell anyone or fill out an accident report. He said he hurt his back on 6/5 while driving a 777A back dump truck hauling rock when he ran over a hole and it jarred his lower back.
On 6/10/08, employee notified his supervisor that he might need the next day off as his back was hurting from helping the UPS driver unload the daily delivery on June 5th. EE was sent to see his physician who has requested futher testing. Currently ONGOING.
Employee was operating a crawler on a dump. Employee exited the cab and attempted to grab a flag on the ground while holding on to the manbasket. The manbasket came loose and the employee fell to the ground.
Miner jumped from the loader he was operating when it began to slide.
The employee stated he was struck in the head by a small piece of rock measuring 18"" x 12"" x 1-1/2"" on the head while standing operating the CM in the #5 entry.
Prying on jack pin, slate bar slipped twisting his right shoulder. He fell to the ground on same shoulder. Began missing work on 10/12/08. He had surgery.

Employee was picking up a copper cathode out of the cell line when he felt a burning pain in his back.
On 6-4-08, employee had a piece of rock fall from the top and hit him on the right foot as he was bolting top and continued to work. His foot swelled up and he started missing work on 6-10-08 and returned 6-17-08.
Employee alleges that he was welding inside a chute and received a couple small welding burns on his legs. He did not report it. One week later he stood up from a desk and felt a sharp pain in his left knee. The next day he was admitted to the hospital with an infection in his knee. He alleges the infection came from the small welding burns.
Employee was helping take a twist out of a fall cable on derrick, when he slipped and struck a glut set up in next line with chest area, causing a fracture to 10th rib on right side.
EE was cutting plastic with a utility knife, the plastic slipped and the knife cut his right index finger. EE received stitches and was taken off work for a few days. EE was wearing his gloves.
Employee was exiting trackhoe. He smacked his hand on the track due to loss of footing because he was in a hurry and was not careful with his stategy of exiting the machine he was operating. Employee was in a hurry because the plant had shut down.
Pushing on discharge pipe on dredge, he strained his back.
Employee was dumping a load when he caught his left ring finger between the seat and the dump lever mashing it. Employee did not report or begin missing work until 6/16/2008.
Employee was moving the lids of a screw conveyor into place with a pry bar. After relocating several of these, the employee experienced pain in his right hip. Late that evening, he made a visit to the local hospital where he was told to rest. He is awaiting a complete diagnosis and has missed 4 days of scheduled work as of 6/10/2008.
