A roof fall was found on the #3 track at the #15 break during a fire boss examination. The fall measured approximately 10 ft. wide, 15 ft. long and 4 ft. high.

The elevator door was left open and the elevator would not come down. The door on the surface had to be closed. No one was on the elevator at the time of malfunction. Elevator down time: 61 minutes.
A roof fall was found by a employee while he was making his weekly, at break 65 in between the #2 and #3 entry in the return. The roof fall is approx. 30'long x 19' wide x 7' thick. The roof fall is not on the active section and is timbered and danger off.
A ground fall occurred in the 4020 - 3950 escape raise estimated 30' long by 20' wide by 15' high at 750 tons. No persons were invovled in the fall. The ground fall would not allow escape raise to be used. A temporary secondary escapeway was implimented.
During shutdown of coal mill, smoke was seen in duct to the baghouse. The Control Room intiated corrective actions. Flooding the system with pressurized inerting gas which resulted in an explosion door being blown open in the baghouse. The investigation shows that sudden pressure within system was likely a result of the inerting system versus an explosion of coal within the system.
EE (fan attendant) was entrapped on the Sugar Run elevator for 80 minutes. No damage to elevator, govenor breaker was re-set.
A rock fall occurred in 1 North Mains in the #4 entry at break #103 extending to break #104. Fall measured approximately 20' wide x 70' long x 5' thick.
During the weekly examination, a reportable roof fall was found in Box Canyon Mains, entry 8, crosscut 0. The fall was approximately 20 feet long, 20 feet wide and 8 feet high.
INTERSECTION FELL OUT, WE SET JACKS ALL AROUND FALL.
A face ignition was determined to have occurred on the 5 right section at the face of the #2 heading inby 65 block.
on 1/28/09, a roof fall was found by weekly examiner. A roof fall is on old 4-wouth mains at matt of 2 left panel in intersection. This fall is in old works and is not blocking airflow or travelways. No one travels this area. Area was dangered off. Fall was approx. 5' high x 30' long x 19' wide. MSHA inspector
A fall was discovered in a return entry of No. 1 mains, 1 break inby survey station No. 1926. The fall measured approx 20' wide X 20' long X 7' plus in height. There were no injuries or equipment involved. Passage of men and ventilation was not affected.
At 4:27 p.m. the Control Room received a report that smoke was coming out of the ""coal rejects"" pile. The production supervisor on shift called the loader operator and the water truck to the site to spray water over the affected pile, to dig out the smoldering coal and to compact this coal. The incident was reported to MSHA at 4:55 p.m.
The intersection of the intake course at brk 19 spad 318 fell in approximately 20' x 20' x 4' due to a coal rider seam in the sandstone.
Lost power to main hoist, resulting in the reprograming of computer system. Power restored, computer reprogramed, hoist resumed normal operation 7:30pm.
The mine lost power due to an ice storm. While evacuating the mine 4 persons attempted to come to the surface using the escape hoist which was operating on generator power. The hoist stopped short about 20' from the surface. The low speed clutch had failed to engage. The hoist was turned manually to bring the 4 persons the rest of the way out. They got out at 11:26 PM.
A fall of roof occurred in the South West Mains at 34 cross cut, # 6 entry. The fall was approximately 40 feet in length x 20 feet in width and the thickness is unknown. Tha area was supported with 6 foot fully grouted roof bolts.
A non-injury roof fall occurred on the 001 MMU in the #7 entry inby SS#410. Size of the fall measured 10' - 12' high x 19' wide x 30' long. No equipment involved. Fall will be cleaned up and additional supports added. Ventilation and travel will not be impeded.
A roof fall occurred at crosscut outby spad #589, #6 entry on #5 belt area. The fall will not be cleaned up. Fall was 30' long, 20' wide, 4' to 5' high. Did not affect passage of men or ventilation.
A roof fall was discovered in the No 2 entry of 1 right off No 2 north at survey station No 1847. The fall measured approximately 40 feet wide (intersection involved) x 50 feet long x 6 feet high. There were no injuries or equipment involved. Ventilation was not affected and the passage of men was not affected.
Fire was reported at 7:19 a.m. at the ""coal rejects"" pile. A loader and a water truck were sent to the site to spray the smoldering coal with water, dig it out and back drag it. Coal exposed to air for extended time periods may spontaneously ignite. Fire was reported to be out by 2:48 p.m., after removing the smoldering coal from the pile and have it compacted.
At approx. 5:30 AM there was a bounce in the tailgate of the longwall located in 7th west which occured as the shear was cutting thru the tailgate. Due to mechanical repairs, the shear was down for more than 1 hour.
A roof fall occurred on the pillar line of the 6- right section-Rock fell on the miner hand.
A rock fall occurred in the 3 East Mains return air course 1 break outby survey station #4189 at break #28 in the #4 entry. Fall measured approximately 20' wide x 30' long x 6'-8' thick.
An unintentional roof fall occurred on the retreat mining South Section. The fall was approx. 20ft wide x 14ft long x 9ft high. The fall was the result of a slip formation in the mine roof that occurred just outby mobile roof supports. The area was supported with 6 ft point anchor roof bolts.
Unintentional roof fall discovered on this date by company and MSHA officials. Fall was located at North West Mains, #2 hdg, 1 block outby spad 4471, block 8-9. Fall measured 20'L X 7'H 18'W. Area previously supported with 6' full grout resin bolts. Area was mined on or about 01/20/2006. Overburden about 260'.
A fall of roof occurred in the South East Sub Main at 13 cross cut, # 5 entry. The fall was approximately 35 feet in length x 45 feet in width x 14 feet thick. Tha area was supported with 6 foot fully grouted roof bolts.
Employee was stuck on Livingston Elevator at 1:00 AM the elevator contractor was called and the employee was able to exit the elevator at 1:50 AM. No persons were injured or equipment was damaged as a result of this entrapment.
The shearer machine had just started out of the tailgate and was grading bottom when there was a bounce at shield #145---142. The bounce moved the tailgate end of the shearer machine towards the spill plates, contacting 2 of the spill plates bending, breaking or shearing off some of the spill plate attachment bolts.
The service vehicle was used for transportation to and from the field. The service truck was unoccupied and parked before the fire incident. At this time the cause of the vehicle fire is still under investigation but is believed to be electrical in nature.
Operation of the Man/Material elevator cage was disrupted for longer than 30 minutes due to a damaged oil filter housing on the hoist.
Discovered a roof fall in the 1st NW Parallel Butt 55' inby station #112. The fall was from #4 intersection to #5 intersection about 70' long, 8'-16' high. Area was bolted with 6' fully grouted & 4' fully grouted & 2 8' cable bolts. In return 1000' from loading section 1st NW Mains. Did not disrupt ventilation or escapeways. Area was dangered off on all sides of fall.
An unplanned roof fall was encountered on inspection in the primary escapeway on first right section. Location No. 5 entry inby spad No. 603. The fall measures 18 ft wide by 40 ft long by 5 feet thick. This fall came partially out into the intersection of the primary escapeway.
PW-15, a point augered several years ago developed a fire in the #2 gas seam. This fire from time to time becomes noticable with smoke etc. Due to the need to travel by truck close to the seam it is important to eliminate by smothering with overburden.
An unintentional roof fall occurred on the above date. There were no injuries and the fall did not impede ventilation or escapeway. This area was timbered off in December due to roof conditions. This mine is currently in non-producing status recovering equipment. The fall occurred in the return, 700' outby the fence. The fall measured 6' high, 20' wide & 30' long.
Around 12:45 AM on January 23, 2009, a kiln push occurred. Hot clinker came out of the cooler and at 1:13 AM a fire was reported on 62 belt stacker 02 that required off site fire services to be called to the plant. The resulting fire burned the conveyor belt, skirting, rubber impact idlers and dust cover.
Roof fall occured on the #8 conveyor belt at approximately 3:30pm. The roof fall is located approximately 50 ft inby survey station 4034. The fall is approximately 20' wide x 50' long x 4' to 8' thick. The roof fall was reported to MSHA and an investigation is being conducted. This roof fall is outby the active working sections.
A 488 scoop was pushing the tail piece on the section, when the battery plug caught fire.
An unplanned roof fall was discovered on a weekly exam on 1/22/09 measuring 60' in length, 19' in width & 8' in height at spad #3031 approx. 3500' from the working face. Entrances have been dangered & breakered off & the area will not be rehabilitated. This was an old return on a panel. MSHA was notified on 1-22-09 & Judy at the Danville state office notified 1-22-09.
A fall of roof occurred two crosscut outby the dotiki IV bottom in doors in the 6th south west sub mains. There were no equipment, ventilation nor personnel involved.
6:00 AM discored fall main line entry #5 spad # 7321; 60' X 19' X 6' high. Neither vent nor passage of persons were affected. Both MSHA and KY IMSL were notified & investigated. Area was breakered off and will be rehabilated.
A roof fall was found at 1 Seam Mains Brk. 26 between 3&4 entry, the fall was approx.5'x20'x100' the area has been dangered and sealed off. The information was called to the safety director at 2:05 pm.
An ignition occurred in #2 Entry of the 22 East section. The flash occurred at the center to the right side of the cutter head and came up over the head. Flash was described as yellow and orange/yellow. Miner was loading its 2nd car after doing parameters. Miner was found to have 21 of 35 sprays working after the incident. Cause was blockage of water sprays by rust.
An unintentional roof fall has occurred in the No. 3 out by entry at No. 79 crosscut in the secondary escapeway. The fall does not impede travel of miners or interrupt ventilation.
We had a roof fall in an intersection in #6 entry 6 break inside in the intake about 35' long x 4.5' thick in the highest place. Rock fall has been timbered off & dangered off. The air has 2 other entries to go up.
An unintentional roof fall has occurred in the no. 3 out by entry at break no. 79 in the secondary escapeway. The fall does not impede travel of miners or interupt ventilation.
6 Ton motor run away on slope striking brake car parked at the bottom of slope. No injuries
At about 7:05 AM while making examinations, a fall was found in a bleeder system on Northwest Mains at Spad # 1333. The fall was approx 6 1/2' high X 18' wide X 20' long. Ventilation was ot inferupted and no one was hurt. This is a bleeder area, is located in #4 Entry.
A roof fall measuring, 80' L, 19' W, and 8' thick was found in the crosscut between 3&4 entries on 4 belt spad 58667. The area developed Oct 07 was 5500' from the working place, it affected no persons. It occurred due to coal and rock streaks above the 5' torque tension roof bolts.
Failure in crosscut and partial failure of intersection at 90 x-cut off old 33 Mains track at 88 sump. There were no persons injured or equipment damaged as a result of this roof fall.
Hoist running slowly in automatic mode. Upon investigation, an overspeed drive fault was discovered. Cycled the power on the hoist control computer, reset the fault, and put the hoist back into operation at 9:10 pm. MSHA notified at 9:04 pm.
The roof in the LW headgate entry deteriorated over a period of time and fell above bolt anchorage.
Two persons were trapped on the elevator from 12:05 to approximately 1:35 due to a fault in the PLC of Cedar Creek Elevator.
Elevator went down at approximately 10:55PM due to ice hitting the cables hooked to the proximiy swith on the 1st floor. Everyone was pulled back tot he loading points on the sections. Elevator was operating at 12:05AM 01/20/09.
There was a non-injury accident at the #10 coal feeder in the clean coal draw tunnel. The belt went off & smoke was observed at app 9:51 AM. Plant foreman could not determine the source of the smoke so he notified Superintendant & the Safety Dept. Contacted the State at 10:16 AM & the MSHA Hot Line at 10:28 AM. The fire was extinguished at 11:30 AM & verified out at 12:02 PM.
Midnight foreman doing fire boss check discovered roof fall. Fall occurred over weekend when no one was working or in mines. Fall was approx. 6' high x 16' wide x 20' long on No. 2 belt inby 1 block where #2 belt started. Structure & belt damaged.
We at Royal Falcon Mine had an unplanned roof fall on 1/18/2009 in the #2 entry at the +770 cross cut in the 1st E. Panel. A Fletcher double boom roof bolter was partially under the fall. A plan for recovery of the bolter was approved by MSHA and the bolter has been recovered. A clean up plan has has been approved by MSHA and clean up is now underway. No one was injured.
Case#1-59923133. Prior to the first crew of the week entering the mine, the service hoist could not be started due to no power on the MG set. Electricians worked on the promblem and the hoist was returned to service at 1:00 am. MSHA was notified at 11:00 pm. No employees were in the mine during the incident.
An unintentional roof fall occurred on the Galatia Mine Galatia North Portal travelway at crosscut 50 4500'. The fall was approx 25-30' long, approx 19-20' wide and approx 6-8' thick. The fall did not affect vetiltation. The fall will not be cleaned up.
While pulling the recovery walker shields forward, a roof/face fall occurred on the B-7 longwall recovery room.
An EE reported a fire at Kiln No. 1 booster heater at 6:01PM. 911 was called and the local VFD responded within 10 minutes. The fire was fully out at 6:35PM. The fire likely started when no. 2 fuel oil leaked from abandoned oil lines onto an electrical coil. MSHA Inspector Richard Moon (Albany Field Office) inspected the site on Tuesday, 01/19/2009.
A roof Fall was discovered along the Buckshot #1 belt-line in the intake. The fall did not affect/impede passage or ventilation. All entries to the fall have been timbered/dangered off to prevent travel.
An unplanned roof fall was discovered in the #1 entry, approx 3000' outby the #1 working section. All enterances were dangered off and this area will not be rehabilitated. This area was bolted with 6' torque tension bolts and 12' cable bolts. The fall measured 60' long X 18' wide X 7' thick.
R-3 refuse belt caught fire due to a faulty bearing and could not be extinguished within 30 minutes. The belt was broken and the fire was extinguished with no injuries.
The elevator serving the #6 portal was unavailable due to a malfunction in the door close switch. It was put back in service at 9:10a.m. on 1/16/09 after the switch was changed out.
A reportable roof fall was found in F-Seam, entry 5, crosscut 72. The fall was approximately 40' long, 19' wide, and 8' high.
# 2 Hoist tripped out on fault, while resetting fault trip, hoistman noticed smoke and fire from DC amp meter. Extinguished the electrical fire on the meter and took meter out of service, repaired wiring to get hoist back in operation. Hoist back in operation at 12:00 PM
An ignition of gas or dust was determined to have occurred on th e5 right working section in the face of the #1 heading. A blue flame was observed by the miner operator approx 4' off the left side of the miner drum.
Man material hoist located in the primary escapeway was inoperable in excess of thirty minutes. MSHA notified at 7:30 PM. Trailing cable was damaged and repairs made to return hoist to operating condition at 9:15 PM.
Lost flame on burner in kiln about 11 hrs into preheat. Operator notified supervisor that he could not verify that he had flame & then deselected the gas panel shutting down gas flow. Shortly after two concussions occurred. The resulted in no damage to kiln, no injuries to personnel.
A roof fall was found inthe return #20 x cut on # 2 belt h 5 entry. it had coal striks in with sand strikiing and soft topic. It was 46"" to 50"" high 20' wide 25' long and it was in intersection.
""Dribble"" was observed in the N12 heading as a worker worked in the cross-cut V24. The ""dribbling"" continued for a period of time estimated at approximately 5 minutes. After that time a section of the roof fell. The material seen on the floor averaged approx. 12""-24"", and amounted to an est. 100 Tns. No damage or impairment caused.
A roof fall occurred in the #9 seam in the East Mains in the intersection of crosscut #23 in #4 entry measuring approximately 18ft. x 18ft. x 12ft high. This area was supported with 6ft. tension rebar bolts, 9ft. meag bolts and 12ft. cable bolts.
Elevator went down at approximately 11:10PM when a piece of ice hit the switch for the 1st floor doors. All personnel were pulled back to the loading points on the sections. The elevator was repaired and operating at 12:20AM.
An unintentional roof fall occurred between 7+80 and 8+40 in the #9 entry in the 7th West Submain (return). The fall did not affect ventilation, passage of men or cause personal injury. The area was mined in September of 2004. The area had been bolted with 4 foot fully grouted roof bolts. The fall measured 35' x 20' x 6'.
Roof fall on section at x-cut 4 from right rib of #4 intersection thru #5 &6 intersections into pushover toward #7 intersection, approx. 100"" long, 19 feet wide, 6-8 feet in height above anchorage, due to horizontal stress. No one injured, no equiptment involved. Area dangered off, section abandoned. Moved to new section location.
Swepco power outage affected city of Grand Saline and multiple counties. Power was restored at 8:31 a.m., the hoist was tested and mining operations resumed.
A roof fall occurred 210 feet outby the working section in the #8 entry. The fall is located 1 break outby survey station #3175. The fall was approximately 50' long x 19' wide x 6' thick.
Elevator at Livingston Portal stopped entrapping two individuals from 2:45 PM to 4:25 PM when they were able to exit the elevator.
Ice that was being chipped from shaft wall fell damaging the wiring for the over travel and slow down switches.
A roof fall occured in a neutral entry intersection approx. 2500' outby #1 section 8 breaks inby sycamore #1 belthead fall measured 25' long, 18' wide and 6' thick. no injuries, no passages of men affected. exact time of fall unknown.
An unintentional roof fall occurred on Unit #2 at Entry #7 at 55+90. the fall occurred due to a slip running through the intersection. The fall area was originally supported with 5 foot conventional roof bolts. The fall is aproximately 15 feet long, 25-30 feet wide, 5-6 feet high. The fall will be cleaned up and resuppported with 6 foot double lock roof bolts.
A short across the lightning arresters on the incoming plant power caused two fuses to blow resulting in a loss of power to #1 and #3 Hoists. The mine was evacuated using the diesel generator to power the auxillary (""Chippy"") hoist.
Experienced intermittent loss of control power due to faulty contactor relay that controls hoist speed. Miners were evacuated and the control contactor relay was replaced. The hoist was tested and miners returned underground at 4:55 pm.
a methance ingition occurred in the face of the 3mno 4 right entry.
Hoist went down at 845 pm due to batteries flat. MSHA was called at 852 pm and hoist batteries were changed and hoist back in operation at 925 pm.
A blasting accident occurred when the 2500 Hitachi shovel contacted an undented primer in an area that had been shot. This accident resulted in no injury to anyone.
The intersection in the #1 entry 1 block inby spad #2259 fell. 21' long x 18' wide x 6' high. Fall area was cribbed & dangered off. It will not be cleaned up.
A roof fall was discovered at 4 Main C. sub-main, 11 x-cut #3 hdg. intersection only. 18' wide 30' long, and aprox. 10' high. No men or equipment were injured or trapped. No ventilation or travel were affected. This fall was in the last open x-cut. The area was bolted with 6' and 8' super twist, and 8 12' post tensioned cable bolts.
A roof fall was found at 11:30 PM on 1/11/2009 between x-cut 45 and 46 entry #2 main south. Fall was approximately 20 x 20 x 7' high. Area had been mined May of 1990 and was supported with 8' full resin bolts. The fall will be cleaned up and supported following clean up plan approved by MSHA district manager.
A roof fall occurred in the 2 sourth 2 mains, #6 entry, between #18 and 19 crosscut. The fall measure approx. 35 feet long, 17 feet wide, and 8 feet thick. The area is supported with 5 foot fully grouted bolts.
A portion of the highwall over the No. 3 portal collapsed blocking the portal for travel. MSHA was notified and MSHA AR investigated and issued a Control Order on this date, Order #7445386.
0230 discovered fall on #1 belt at break #32. Fall 60' X 20' X 7' high. Ventilation not affectd, passage of people not affected and area breakered off. Both MSHA and KY OMSL notitfied and both agencies investigated.
Drum brake would not release. Reset voltage to correct problem.
IGNITION: The Miner was cutting in the No. 2 Entry on the No. 6 Section. The Miner Operator had cut the coal and roof and when he shearred down and touched the mine floor, an ignition occurred on the right side of the Miner. It lasted approximately 5 seconds and the water sprays on the Miner pit the fire out.
A roof fall measuring approx. 17'x50;x20+' occurred 1 x-cut inby spad No. 121. The fall area was dangered off and the area abandoned. The fal will not be clean up.
An examiner found a roof fall in the 4 Main North intake in entry #7 between crosscuts 166 and 167. The fall was located in the entry adjacent to the primary escapeway. The fall was not located in a crosscut, but was present in a pillar line.
A roof fall was discovered at 3 Mains Section, #10 Heading, 55 to 56 Crosscut. 80' long, 10' high, and 17' wide. This fall was 2,600' out-by the Mains Section. 6' super twist and 6' fully grouted resin bolts were in use. No men or machines were injured or trapped. No ventilation or travel were affected.
Eevator malfunctioned at approx. 2:45 am. Electrician trouble shooted problem and discovered top limit switch needed adjusted. Adjusted switch and elevator started working properly.
Roof fall last open cross cut between 3&4 entries of 1-right sub-main in the primary emergency escapeway, partially blocking escapeway. Did not affect air. No one injured and no equipment involved. Area cleaned, rebolted and prop-setter posts added for additional roof support.
A roof fall was discovered at 3 Mains Section, 53 Crosscut, 10 to 11 Crosscut only. 9' high, 17' wide, 20' long. 6' super twist and 6' fully grouted resin bolts were in use. This fall was 2,600' out-by the mains section. No men or machines were injured or trapped. No ventilation or travel were affected.
A roof fall was discovered at 10:40 am in the Second East Mains, #3 entry, X/C 34. The fall measured approximately 25' wide, 25' long and 8' high. The fall did not affect travel of persons or any ventilation current.
A blasting accident occurred when a mud seam blew out sending rock off of the mine site and onto private property in the Fedscreek Community of Motlex Fork. No injury resulted from this accident.
The roof between the last shield canopy and the first row of bolts in the tailgate, fell out making it unsafe to travel off the longwall face into the tailgate. The tailgate entry was not affected. The bad roof measured approximately 3 ft wide by 3 ft high and 6 feet long. The face retreated past it after two passes.
An unintentional roof fall occurred at the Galatia Mine Galatia North Portal on the Main North #1 Entry 8175' to 8230'. The fall was approximately 35-40 feet long, approximately 18 feet wide, and approximately 8-9 feet thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up.
A roof fall was found in the old works at the Short Block Mains in the #4 entry, the fall was approx. 20'x30'x5' wide, the area has been dangered off.
An unplanned roof fall at the anchorage zone where 6 foot bolts were in use. The fall is approximately 15 feet long by 10 feet wide from the solid rib toward the center of the intersection of 7 Main North, #1 Entry at Crosscut 72 (71+78). It appears fall reached the anchorage zone at only one bolt in the fall area. No apparent contributing factors were identified.
Fall discovered 6:55 AM, break #18 at old 2A blet in return #2 entry. Fall 20' X 40' X 4-5' high; area was breakered off. Both MSHA and KY OMSL were notified, neither investigated. This was a continuation of a previous fall.
A methane ignition occurred on the 003-0 MMU (002 Section) in the face of the number 4 heading approximately 160 feet inby spad # 11072. Located in the 2nd LT. Panel off of the No. 5 Southeast Mains.
An unplanned roof fall occurred on 1st South East submain at cross cut # 31 in # 4 entry. The area was roof bolted with 6' and 10' bolts. The fall appeared to be fractured stack rock. The fall will not be cleaned up.
A fall of roof occurred in the 1st East, entry # 6, station 26+30. The fall measured approximately 30 feet in length x 40 feet in width x 10 feet thick. The area was supported with 6 foot fully grouted roof bolts.
Fall discovered 6:55 AM, break #18 at old 2A belt in return #2 entry. Fall 20' X 40' X 4-5' high; area was breakered off. Both MSHA and KY OMSL were notified, neither investigated. This was a continuation of a previous fall.
A fall of roof occurred in the South East Return at 49 cross cut, entry # 2. The fall measured approximately 30 feet in length x 30 feet in width x 6 feet thick. The area was supported with 6 foot fully grouted roof bolts.
An employee had traveled to the refuse dump with a load of rock when the truck engine compartment caught fire. He exited the truck with no injury. He called for assistance and the fire was extinguished. The truck cab sustained substantial damage.
At approximately 11:35 am the belt went off in 12th West. A roof fall was found at 21 x-cut in the belt entry that measured 10'L x 20'W x 4'-4.5'H and had cut the remote line shutting the belt off. The area was dangered off and federal inspector Donnie Durrant was notified on the property.
An unintentional roof fall occurred on #9 belt #2 Ent (Neutral) inby spad # 10235. Support being used was 4' resin bolt. Approx length 20', width 20' and height 5 1/2' No injuries occurred.
Case number 1-59610573. South brake on production hoist experienced a set fault error that could not be reset. Mine was evacuated at 6:20 pm and MSHA notified at that time as well. A loose connection was finally discovered and the hoist was put back into operation and miners returned to work at 7:20 pm.
A roof fall in the 6th West bleeder 100' inby MPL #1 at x-cut 3-4 was found by a weekly examiner and reported to the Safety Supervisor. Shift foreman went to determine if it was reportable and it's size. Reported to be 20'W x 60'L x 6'H. Five foot resin grouted bolts were used in this area. No injuries, not impeding travel, or blocking escapeway.
A roof fall measuring approximately 20' x 30' x 15' occurred 1 x-cut outby spad #149 between the #3 & #4 entry. This fall is approximately 40 x-cuts outby the active section in an area where men are not required to work or travel. The fall area has been dangered off and will not be cleaned up.
While examining a de-watering pump 3 cross cuts inby syrvey station 213 the examiner found a roof fall at an intersection 1 cross cut inby survey station 213. Heintzmann's were set at each openings to the area. Fall was in an intake air-way. Travel and escapeway have not been compromised in the area.
Hoist car jumped off track at track switch on surface. The hoist went off the track at 12:56 am and was back on track at 1:52 am MSHA was notified at 1:11 am.
A fall of roof occurred in the 1st Left off the 1st Right in entry # 0 between 10+90 and 11+80. The fall was approximately 50 feet in length x 19 feet in width x 10 feet thick. The area was supported with 6 foot fully grouted roof bolts.
An unintentional roof fall occurred on the longwall tailgate at approximately 3:20 PM at intersection #79. The roof rock is estimated to be 15 to 20 feet long and 6 to 8 feet high. MSHA and WVMHST were notified timely and the longwall tailgate blockage plan was implemented.
Roof fall occurred in the 5th south eat butt belt line starting 2 1/2 crosscuts inby ss# 5458 and extension approx. 100 feet.
We at Royal Falcon Mine had an unplanned roof fall in the 1st East panel at the +580 cross-cut in the #6 entry inby the section tailpiece. The roof fall fell on our Joy 12 continuous miner. All rehab plans have been approved and been followed. No one was injured in the fall.
Discovered a roof fall in the GL5 panel 1 break inby spad #2791 which is 3000' from the active section. It was developed on 6/2008 and bolted with #5 x 48"" fully-grouted resin bolts. This area has been dangered off.
During a weekly examination of the intake airway, a rock fall was discovered 2 breaks outby survey station # 3065. The fall was in the #5 entry of the 003/004 mmu outby area. The fall was approximately 30' long x 30' wide x 7' thick. The fall was in an intersection.
Rock fall #1 occurred on 1 East Mains in the #1 crosscut between #3 and #5 entry. Fall measured approximately 20' wide x 60' long x 4'-5' thick. Rock fall #2 occurred on 1 East Mains in the #2 crosscut between #6 and #7 entry. Fall measured approximately 20' wide x 20' long x 3'-4' thick.
At 10:27 AM ee reported a roof fall at crosscut 30 between #6 and #4 entries, taking out intersection in #5 entry.
An unintentional roof fall occurred on the 4C at station 22+40 entry #1. The fall measurd 93 feet x 20 feet x 8 feet. Since the fall was located 2 entries over in the return from the examiners route this is not a reportable fall. approximate date of developent was 10-15-2007. It will not be cleaned up.
A raw coal pile of approximately 100,000 tons had been stockpiled over a period of many months. The fringes of the pile, because of a lack of compaction, became hot and eventually started to burn throught the crust of the pile.
The #3 coal mill shutdown automatically when a high temperature occured. The coal mill was loaded. After approximately three hours the coal mill was started to try to empty the coal when an unplanned ignition of dust occured resulting in the two explosion vents opening.
Fire started in dust collection duct work off of line 3 in our coloring dept. Fire lasted about 40 minutes. Fire Dept called. No injuries
A fell of roof occurred in the south east lift of mine C at x-cut 512. Ventilation, personal nor Equipment was invlved or effected.
An Unitentional roof occurred on the 4B at station 30+20 entry #10. The fall measured 38ft x 20ft x 6ft high. Since the fall was located 2 entries over in the return from the examiners route this is not a reportable fall. Approx date of development was 1/28/06, it will not be cleaned up.
During an exam, a roof fall was discovered on this date. The fall measured 19' by 65' and was 6-8' in thickness. The fall was located at 3-North-1 #10 crosscut between the #1 and #2 entries. The fall was cribbed off. The area was previously supported with 5' HY-Tech bolts.
The mine encountered a roof fall at 19+25, entry #5, of the Main Northwest. The fall measured 4'L X 18'W X 4' thick. The event was called in at 7:30PM. 103(k) issued at 7:55AM on 1/5/2009. The area was developed 7/10/2002, bolted with 12' point anchor and truss bolts. The fall will be cleaned up.
An unplanned roof fall occurred on the #2 section #3 entry in the return at spad #5660 5,500' from the face.
An unintentional roof fall occurred in the old 5D worked out panel (Return) at station 10+30 between entries 1 and 2. The fall measured 50 feet x 19 feet x 6 feet high. The fall did not affect ventilation, passage of men or cause personal injury. It will not be cleaned up. The crosscut had been bolted with 4 foot fully grouted roof bolts.
An untintentional roof fall occurred between crosscuts #1 and #2 in #3 entry on Headgate #2 set up rooms. The fall was 60'Lx18'Wx9'H.

A roof fall wis discovered by the mine examiner at spad #6455 approximately 2600' outby the faces. Fall measured 25' long x 19' wide x 5' high. No personnel or equipment were involved. Area will not be rehabilitated.
A non-injury roof fall was discovered at approx. 7:00am at the #17 cross cut, #4 belt return, #5 entry. The rock fall did not impair the ventilation. The area was timbered off and dangered off. The brattice line was moved over to the # 4entry to have a safe means of traveling the return. The fall measured approx. 19 ft x 19 x from 2 ft to 8 ft high.
A fire ignited in D10R bulldozer at rear of engine near front of cab, destroying the cab and its contents.
Hydroblasing nozzle exited pipe striking employee.
Employee entered area of accident by foot deviating from procedure. Was found later in bottom of hopper engulfed by stone.
The victim was operating a skid steer loader underneath a belt conveyor that was being dismantled. A co-worker was in an elevated manlift & was using a cutting torch to remove a piece of 4"" metal tubing attached to the leg supports of the belt conveyor. The tubing fell to the ground, tipped over into the front of the loader & struck the victim. He was hospitalized & died on 1/9/09.
Employee was checking the heater in the #2 Euclid haul truck to see if it was blowing out heat. The employee's foot slipped and to catch himself he grabbed the heater unit with his hand and his finger went into the rotating blower fan and cut his right ring finger.
While operating the roof bolter, Ee got his hand in between boom arm and drill pot. This pinched off the tip of his middle finger on his right hand.
EE was operating the AL Lee forklift, he placed his left hand on the outside of the canopy to pull himself up in the seat, when he did he accidentally hit the reverse tram pedal. This caused the forklift to move backward, smashing his left hand and fingers into a roof bolt plate. Ring finger couldn't be recontructed and was removed, middle finger expected to recover at this time.
Closed door on loader and cut end of rt index finger off.
Employee was installing a fan and pump on 992 G wheel loader. Fan and motor was sitting on machine while chain was bang repositioned. Fan and motor began to fall knocking him to the ground. the fan fell on the right foot. A blade from the fan cut throught the steel toe on his boot severing his big toe.
While installing roof bolt a piece of rock came loose and smashed finger on bolting machine frame rail.
Employee was operating a drill and had opened the door. When he closed the door he caught his left index finger and thumb in the door. Left thumb was amputated from the lack of his fingernail (tip)
The subject was moving a shield in manually. He knelled on the toes of the shield with his feet hanging off. He moved the shield towards the pan line. His right foot and left toes were crushed against the pan line and the toe of the shield.
EE was working on the ground when a beam clamp fell from the 4th floor & struck him in his head. He did have all required PPE. The clamp was being removed by employee on the 4th floor
Building stoppings in return. Rock fell from top, hit him in head and slid down and hit back.
Employee was roof bolting. As he was trying to place the roof bolt in the head of the roof bolter, he inadvertently bumped the rotation lever, thus catching three fingers on the roof bolt as it was rotating.
Employee #1 was clearing frozen material from the road runner feed hopper. Still not understand the grizzly on the end of the feeder began lowering. Two employee who were just a few feet away, immediatly warned the grizzly. Both ee stated that #1 ee wouldn't respond or react 10 to 20 second. he was pinned by the grizzly. due to his injuries. the investigation is ongoing.
Climbing up tarp rock, EE slipped and fell and stubbed his right hand.
EE had his right index finger on the pot area when he raised the pot to install a roof bolt. His finger was pinched between the drill pot and the roof bolt plate.
Employee was making repairs replacing parts and a chain was used for lifting. He was pullling chain over where he was working and strained his left knee. EMPLOYEE DID NOT START LOSING TIME UNTIL 12/3/09 DUE TO SURGERY.
EE and another member were changing a tailshaft on a C/M. EE had one end of the tailshaft in place, while setting the other end the tailshaft slipped out and fell striking the middle and ring fingers on left hand.
While striking a belt knocker with a hammer, a piece of the hammer chipped off striking the employee in the left wrist.
Ee was exiting r-motor, he placed his right foot on the step and when he swung his left leg around he lost his balance and fell to the floor. He felt something snap in the right knee.
Employee stepped in a hole on the belt line with his left heel causing it to get stuck in the mud. When he tried to step forward his knee locked up, hyper-extending and straining his left knee.
EE stated that he stepped in a rut and twisted his right knee. EE did not report this accident to his Foreman or anyone else on this Friday. On Monday he called in and said he had car trouble, then later, when questioned, he said he hurt his knee. We feel this was a home accident and not work related.
Employee seems to have fallen out of his loader following fueling. He does not remember what happened, but evidence at the scene indicates that the nozzle was left in the tank as he drove away. Injuries to the employee include broken ankle, shoulder, ribs, nose, and facial fractures.
It started snowing fast, employee was going down hill when his truck started sliding. He turned a 180 and turned over on its side. He got out himself.
Employee injured his left shoulder while putting up rib boards. His first day missed due to the accident was 3/20/09.
The ground was muddy around the track, the employee said that he slipped on the track rail and hurt his leg. Right thigh, there were no wittnesses and the accident was not reported until midshift.
End loader bucket struck the side of rock truck bed while truck was being loaded. Note: Injured did not seek medical treatment until 2/13/09.
Employee was moving a light plant. A bar slipped and hit employee in ribs
EE reported: Operating roof bolter in face area. When attempting to position the head of the roof bolt into the rotation chuck of the drill head, spun the bolt. Unknowingly the bolt had a small bend, when it spun this bend caused his wrist to be bent backwards, resulting in a hairline fracture to the ulna bone in the wrist.
Employee was fueling unit #143 (Picker). When employee stepped down off of track, employee's left ankle rolled on slippery surface, causing strain.
The piece of rock he had just split fell to the side. While attempting to upright the stone to continue splitting, he strained his lower back. (He was wearing back support.)
Employee was moving a water pump to deeper water and strained his back.
EE was carrying an 8' timber in the crosscut between #1 and #2 entries, when he stepped into an uneven place in the bottom, causing him torn ligaments in his right knee.
EE was moving a barrel of fluid, causing pain in his abdomen.
Injured was using a miner bit to drive a broken miner bit from the bit lug when he struck his left index finger with a ball pien hammer. He was transported to the hospital where he was dianosed with a fracture and laceration to the left index finger.
Lifted garbage bag in administration building, strained back.
Adjusting sandscreen replacement that was being held in place by loader. Strained back by moving screen manually.
Employee was mucking in a line with a LHD with a cab. As the employee moved forward a rock came around the front of the bucket. The LHD wheel ran over the rock and jarred the employee resulting in a back injury.
Sustained an abdominal injury (umbilical hernia) while reaching high into wheel group of a Komatsu 830E haul truck while standing on a ladder to remove a wedge which weighs approximately 15 lbs. Surgery performed on 29-Jan-2009 to repair the hernia.
The injured miner stated that he & 2 other miners were hanging a 6 inch discharge plastic water line. They had it hung 3 times & when they picked it up to hang the 4th time the other 3 hangers broke. The water line fell and hit him on the elbow on the left arm and bruised his arm and caused pain in his elbow.
Employee was driving a motor and it jumped track hitting left rib causing injury to the employee.
Injury was walking under a belt. He stood up and walked into a second conveyor, hitting his face on the metal frame. The bridge of his nose was injured.
2 employees were attempting to lift a compressor. It was too heavy so one tried to slide it by pushing it. He felt a sharp pain in his groin that shot up his back to the left side of the neck.
While trying to keep the fuel pump motor running, employee had to climb onto Fuel Truck to start. Employee slipped and fell off back of Fuel Truck onto his right side. There was a significant sleet accumulation and freezing rain during this shift. Employee was seriously injured. Fractured Collarbone, sternum, 1 fractured rib, 2 fractured back vertebra.
Employee pulled on a concrete form to strip it from the concrete. He did not report that he was in pain at the time. He worked his shift and returned to work the next day and worked. He was in obvious discomfort and was required to seek medical evaluation. The doctor said that he had a strained or damaged rotator cuff injury and laid the employee off for 2 weeks.
While walking along near a tail pulley, the EE slipped on some ice and fell. He received medical treatment on 2/3/09.
Employee was getting off track loader when his foot slipped on the track and twisted his left knee.
Employee was helping unload a hydraulic jack from the back of a service truck. He pulled the front of the jack out of the truck. Before other employee could grab other end the jack fell out of the truck with the injured employee holding one end. After 3 days of physical therapy his back is better.
Went to Landmart Gas Co. to get a propane tank. Employee helped the supervisor lift a 100lb propane tank and carry to truck. The employee complained of lower back pain, but indicated he could return to work. He continued work, handling stone and buidling sample stone panels. Later in the shift he experienced severe back pain and had to be taken to hospital.
Just starting shift. Started to wet down stops, grabbed water hose, turned and twisted left knee.
EE was loading truck, had stabbing pain in back. No one witnessed the accident.
Employee states that he was dismounting from his Cat D10R Dozer when his feet went out from under him and he fell to the ground onto his right shoulder. He believes that he may have slipped on snow and ice on the tracks/structure.
Miner was hanging high voltage cable on a ladder with two co-workers holding the ladder. The cable fell, knocking the miner off the ladder.
Had rock bar, trying to pry track off cable.
Employee removed steel plate from crusher. Plate dimensions 5"" x 12"" x 3/8. Employee turned to lay plate down, accidentally dropping plate on other employee's left forearm causing forearm contusion.
The employee was installing a roof bolt when a piece of rock and coal fell striking him on the back.
Employee tripped on wire and fell, causing fracture of fibula in right foot and required surgery.
While working outside, ee walked across an icy area. His feet flew up & he fell flat on his back knocking his hard hat off. He laid very still & possibly lost consiousness, but got up & continued working to finish the day. The next day was a snow day off & he continued having headache & soreness. He sought medical treatment on 1/29/09 & will be off work until 2/2/09.
Employee was helping change a battery on a man trip when battery lead arced and a spark struck employee in face & eye.
Employee put in the last outside bolt. He raised the jack and a rock fell off the canopy and hit him on the head and neck.
Employee picked up metal bar and placed on his shoulder, this resulted in a lower back strain.
Employee was working on snow removal with his highlift when he struck a piece of frozen ice and it jarred him in his machine causing pain in his lower back. EMPLOYEE RETURNED TO WORK ON 1/28 & 1/29/09....FIRST DAY MISSED 1/30/09
Employee states that while lifting a sieve, he pulled a muscle in his left shoulder. (Started missing work on 3/5/09).
injured dislodged a rock from the tailgate of a coal truck when it shut injure left ring finger got caught between the tailgate and the bed of the coal truck. He was transported to the hospital where he was diagnosed with a fractured and lacerated left ring finger.
Employee was positioned near a coal rib, while the operator of a dual boom roof bolter was moving the machine. The roof bolting machine moved suddenly to the right resulting in contact with the injured employee. Injured employee is currently waiting to see a specialist.
While doing maintenance on 990 Cat front end loader, employee slipped on ice and fell down the stairs on back of loader hitting head and fracturing right ankle.
Injury states he was driving his coal truck on the haul road when he started sliding backwards, even though he had chains on. He slid into a ditch.
Working on sand atacker, employee fell approximately 24' into sand dome and injured his left arm.
While trying to dislodge the frozen pulling lashing from the loaded barge, something ripped loose and popped in my right shoulder socket.
Injured was carrying a 6"" flange weighing approzimately 11lbs down the strairway between the 5th and 6th floor when his ankle gave out causing him to stumble striking his face against the handrail. Injured was transported to the hospital where he was treated for a laceration above his left cheek and eye.
Employee was shoveling spilled material on 1/26/09, on 1/28/09 he developed a lump on lower left abdomen.
Employee was operating a 789 Rock Truck and was about to be loaded. A 992 K Loader pulled up to dump into the bed and the loaders front tires encountered a mud seam and three rocks fell out of the bucket and inside the truck bed, jarring the employee. He was diagnosed with a back strain and was scheduled for an MRI on 02-13-09.
Employee slipped on rollers while walking on overcast, striking his left knee when he fell. Employee did not start to lose time from this injury until 02-15-2009.
While tramming diesel mantrip down slope track, brakes failed causing mantrip to derail and wreck at bottom of slope. Employee was thrown out and complained of back pain.
Employee was walking from his personal vehicle to the rock truck he was going to operate when his feet slipped out from him and he fell landing on his right leg.
Injury slipped on the snow and fell. As he was falling, he grabbed for a pallet to catch his balance. A stone fell from the pallet and smashed his finger. He was taken to the clinic the next morning. An X ray was positive for a small fracture in his distal phalax of the first digit of the left hand.
This is being filed within 10 working days from the date that the accident became reportable. Employee reported that he slipped and fell inside of bed of haul truck while welding liner plate. He initially declined medical treatment. On 01/31/09 he complained of back pain and was exmained by physician who diagnosed a back sprain and placed employee on restricted duty.
Employee was pulling highline to get slack out of the line. Upon pulling the highline, employee felt a sharp pain in his lower back. MRI results were not read until 2/16/09 causing his first missed shift of work to be 2/17/09.
Employee was lining up a lift cylinder to a D-block on a D-11 Cat dozer. During lining up of the cylinder and the D-block. The employee's left hand was caught between the D-block and the gusset on the dozer blade resulting in fractures to the left middle and ring finger.
Employee tripped on a T-Board going to the Roofbolter to move it. He DID NOT START LOSING TIME UNTIL 5/21/2009.
Employee was greasing bulk truck and strained his back.
Cleaning belt tailpiece, fell on belt, went down belt & hit head on roller.
The employee was operating a ram car when he hit an arch causing it to break loose coming down on the employee. The employee received 2 cracked ribs.
Pulling on slate bar to shut doors on railroad cars and felt a twinge in lower back.
Injured was operating miner standing against right rib who's shuttle car backed out of place and stopped in slid against rib catching injured between rb and shuttle car.
In the rain, employee was putting Hyd Oil in a D-11-R Dozer when he slipped on the dozer's tracks causing him to twist his ankle. He completed the remaining 15 minutes of his shift. Later that day employee went to the Doctor and X-ray revealed a chipped bone in left ankle.
Employee was mounting his truck after checking the oil filter for a leak. The employee slipped and caught himself with his left arm, causing a burning sensation in his left arm and shoulder. Employee was diagnosed with a strain by a specialist and will follow-up on 02-17-09 with the specialist.
Injured bent over to move rock and felt pain in his back. Strained lower back.
He was unloading rails from a car, when his left index finger was causght between tow rails, causing a fracture and laceration requiring 8 sutures.
EE was getting a bag of rock dust (weight 40lbs.- 50lbs.) off the scoop and as he lifted on the rock dust he felt a sharp pain in his back.
EE had a scratch on his hand and noticed that it had a sore on it. He broke the sore open and thought it was fine. The next day his hand was swollen when he got up. Does not know when he scratched his hand.
Injured was working on drill head of roof bolter when a rib rolled off striking him on the back of his right ankle. Sprained ankle.
Employee pinched finger between concrete form and vent tube.
While operating a 4 seat mantrip, injured claims to have run over a rock in the roadway. When he ran over the rock, he claims to have felt pain in his back. He continued working until 1-31-09, at which time he went to the dr and the dr removed him from work for 3 days.
Injury was working on hard build up inside the waste dust silo with a chipping gun (most of the work above his head due to the location). He had been working on the build up for about 45 minutes. After he stepped down for a break, he informed me that his right shoulder was starting to hurt.
miner was operating a ram car on a grade. the miner did not put the ram car into first gear and it gained speed. the miner rubbed the rib with the ram car to slow it down and was jostled around.
Injured party had just rolled a piece of rock off the belt. When he tried to stand up, he felt a pain in his lower back.
As employee was investigating plugged temperature probe on preheater tower, hot cement puffed out of port entering employee's eyes (was wearing protective clothing including face shield).
Employee was dismounting a ""lizard truck"". He missed the last step causing him to fall.
Injured was pushing the insert back into the head when he pushed the lever in the up direction catching his right little finger between the steel and the chuck. He was transferred to the hospital where he was treated for a lacertaion to the right little finger. No stiches were used.
Injury was a contusion below left knee, which occurred while employee closing sectional elevator door. Employee was returned to full duty date of injury. However area started to swell and discolor, so employee when back to doctor on 2/3/09. Doctor lanced the bump and inserted a drainage tube. Employee then placed off work as of 2/3/09.
Hanging curtain up, got bumped by shuttle car.
Employee stated that he had lifted a piece of rock off the line curtain when another rock fell from the mine roof striking him on the top of his right foot.
While walking to his monitor in the pit he slipped and twisted his knee. The incident just became a reportable/lost time yesterday (5/4/2009) when it was found out that his injury requires surgery.
While helping put bottom roller in place, employee alleged that he strained a muscle in his right upper chest.
When removing existing power poles, sandy soil along with poor placement of crane caused rear outrigger to giveway when pole was removed. When the outrigger pushed into the pole hole the crane became unlevel & overturned. The operator was attempting to get clear of the overturning crane when he fell from the crane hitting ground causing sprains to neck/back/shoulder
After final receipt and review of medical records on 2/19/09 this incident was determined to be reportable. On 1/22/09 this employee was pushing on a bent roof strap when he reportedly experienced a flash in his right eye and pain in his back.
EE was trying to pull down a piece of ventilation curtain. When he jumped up in a pull down motion. When he landed on his feet, his left ankle twisted and popped. He was transported to the hospital where an x-ray showed a fracture.
Employee was initially injured on 6/19/2008 (E-Docment #638715). Employee has been back to work full duty. On 1/22/2009 he was taken off work again (no new injury). A new 7000-1 is being submitted as per the MSHA definition of a pre-existing condition.
Employee was removing access door from cement mill, door fell (approx. 100#) trapping gloved finger between mill and bolt. This caused severe laceration to left index finger.
Employee exited the mine and was walking to the bath house, when he experienced severe pain in the lower middle of his back.
Miner was walking down face line and walked into shield #133.
Location in Mains. EE was changing rearend in a rubber tired mantrip. The mantrip fell of of blocks, landing on right leg. He finished out shift before going to the Montgromery General Hospital ER.
EE was setting a 6"" beam on a new overcast wall, the beam caught a high spot in the wall causing the beam to roll over striking the EE's right hand and fingers. The EE's right hand(ring finger) received a severe cut from the palm to 2nd joint of finger. Sutures were required to repair cut.
While clearing material, the operator pulled the bucket of the excavator too close to the cab and a rock (about the size of a softball) came through the windshield of the excavator striking him in the pelvic area.
An employee drove a Track Hoe over a high voltage cable damaging the cable and causing an electrical shock to the injured employee.
The employee had been hanging a curtain when he went to walk off he felt pain in his lower back and right leg. The employee did not start missing work until Sept 8, 2009
Employee stated that he was walking and throwing miner cable out of the roadway when he stepped onto the ledge of a hole in the mine floor causing him to hyperextend his right knee.
As employee was finishing a belt splice, the come-along being used to pull the belt together slipped and struck him in the left shoulder.
Employee was roof bolting at the face when a rock fell from the mine roof and pinned him against the drill head. The employee was taken to the emergency room. He was found to have bruising and swelling in his right hip. Began losing time as of 1/31/09. *Employee began losing additional time as of 3/30/09*
Employee was putting scoop on charger and dropped batery lid on his hand.
The employee states that he was lifting a piece of iron and hurt his back. On his next shift, 02/03/2009, he was in a uniloader and when he went to get out, he felt pain in his lower back. He received conservative care and never missed any work until he required surgery on 03/09/2010.
EE was walking in the #2 entry on #1 section, when he stepped on a rock, causing his left knee to twist. EE continued to work until 2/13/09. After EE had an MRI he was instructed to be off work.

Employee was retrieving a hot meal sample, when an air cannon discharged 30ft below the sample line. This forced some hot air and dust back into the line and out of the opening. The employee was wearing all ppe but the dust still managed to travel under his faceshield, which resulted in his eyes being scratched.
The employee says he had tingling going down his leg in the afternoon while he was operating a loader in the pit. Didn't report anything to his supervisor except on a voice mail late at night.
Jumped off the back of a truck to the ground, instead of climbing down.
Injury was changing out the stationary roller on the #4 Hornet Mains take up . As he attempted to place the shims underneath the bearing as it was being held up by another person with a slate bar, the bar slipped out causing it to fall on his left thumb. The injury required surgery to repair the damage.
EE reported: He and a coworker were bolting the face on #1 unit. The length of bolt being used was 8' long. Each bolt had to be bent prior to inserting into the mine roof. As he was attempting to bend a bolt, his left shoulder popped, resulting in severe pain to the shoulder. Started missing scheduled work days 1/28/09, became a lost time.
While opening loader door, it stuck and employee pulled something in his lower back.
The employee was out of the shuttle car cleaning out the deck when he was hit by a piece of coal in the back pushing him into the shuttle car injuring his right knee. Was diagnosed on 2/10/09 by a specialist after a MRI with a hyper-extended knee and remains off at this time.
EE was putting out a fire on the coal conveyor.
EE was attempting to lift the bottom belt to change out a bad bottom roller, when he felt a pop and sharp pain in his lower back
The loader operator was walking over to the screen to shut it down and a rock, approximately 12 inches by 4 inches, from the grizzly bars on the feed hopper slid off grazing the operator's hardhat and striking him in the right foot. According to the MD the operator has fractures of tarsal and metatarsal bones.
Miner was rebuilding cone crusher during winter maintenance. He put his hand between top part of crusher, which was hydraulically suspended, and the guard. The crusher was lowering down when the miner struck his lower arm between the crusher and the guard and it got pinched.
The man was pulling on conveyor belting when he felt a pain in his left elbow. The man continued to work his regular duties until 4/8/09 at which time he reported off regarding the injury.
This employee stated that he had his right hand lying on the edge of a shark tank to brace while he held a loop of bull hose with his left hand. The hose slipped and swung over against his right little finger pinching the tip off.
DID NOT START LOSING TIME UNTIL 3/25/2009. While squating at dust pod, twisted left knee. Surgery required.

EE sustained an ankle sprain unloading rail cars on 1/21/09. Over the past 6 months he experienced minor discomfort with the ankle. On 7/23/09 he was reviewed by a doctor and no abnormalities were found. He was seen by an orthopedic specialist on 8/7/09. On 8/10/09, The orthopedic doctor gave him lifting and walking restrictions and prescribed physical therapy.
EE was cleaning a roller with a piece of angle iron. A frozen piece of mud caught the angle iron, kicked back and mashed his hand against the structure. X-rays revealed no broken bones but Ee missed one day of work.
Miner was shoveling track entry. 6"" waterline busted getting dust and dirt into eyes.
The employee stated he was walking outby the No. 5 entry and as he passed a scoop that was parked in the entry his foot slipped in a hole in the mine floor that was covered in mud and water causing his foot to twist as he fell to the bottom injuring his left foot and ankle.
Was unloading roof bolts out of scoop bucket when he stepped into scoop bucket and slipped and fell against roof bolts in scoop bucket.
Stepping off 375 dozer, stepped on a rock and his toes hyper-extended causing contusion, sprain and a possible fracture.

The blaster helper was driving Kenworth Truck up steep grade at mine site. The back wheels started to spin & bounce, causing drive shaft to break. Due to weight of material on truck, truck started sliding back down the steep grade. Blaster helper jumped from truck & sprained his left ankle. Which will result in the blaster helper being off work for 3 scheduled work days.
EE was running the grader in the members parking lot, when he saw another member fall in the parking lot. EE then stopped the machine and went to help the member, he then slipped also. Both men went back to work and finished the day. EE missed the next two days and called in stating he had a broken hip.
Employee was taking plug out of air tank. Pressure pushed hand sideways away from the tank into the housing, breaking his pinkie finger on his right hand.
Ee was unloading rock dust from a belt line and twisted his back.
He was standing in front of scoop guiding the operator to load a bundle of roof bolts when his right foot was caught by the bundle of bolts causing a strain to the right foot.

Employee was disconnecting frozen water lines from stockpile. When he was coming back down the face of the stockpile the employee slipped and slid approxmately 20 feet to the bottom. He called the office with his cell phone to tell that he couldn""t get up he had hurt his back.
While moving the roof bolter inby to bolt the next row, employee was struck on the top of his head by a piece of rock that fell from between the bolt and rib. (2.5'L x2.5'W x 3""T)
EE stated that he was rockdusting around the blower area and stepped on a rock. When he put weight on his leg, his ankle buckled and while trying to balance himself, he heard a pop and had pain on the side of his ankle. The rock he stepped on measured approx 1' long X 6"" wide X 4"" thick.
Scoop operator leaned forward over controls in deck to check contents of bucket. The scoop was energized & began moving, mashing torso against mine roof. Canopy is not required in area where seam height is between 40-48"". Employee received fractures to his eighth and ninth thoracic vertebrae.
Employee was moving a 1"" frozen water hose.
While hanging miner cable, Employee lost his footing causing him to slip and feet a stinging sensation in his groin area.
Employee slipped and fell while pulling on the power cable to a shovel. He was evaluated by a Physician and started Physical Therapy on 1-30-09.

At approximately 5:30 a.m. the employee was dismounting from a haul truck. The ladder was wet from snow. He lost his grip and fell approximately 15 feet to the ground. He was transported to the hospital. His diagnosis was a dislocated finger and compression fracture of the L1 and L3 vertebrae.
Employee was in the grease truck when it slid over an embankment and turned over hurting his lower back.
The employee stated that he was trying to pull a roof bolt off of the tray of the roof bolter. The roof bolt was stuck, so he pulled harder. He then felt a sharp pain in his right shoulder. The employee started missing work on 2/4/09.

Employee dismounted ride and placed his body in front of the johnson ride to hook a chain to the front axle when the ride started moving forward causing the employee to be struck by the ride resulting in fractured ribs.

Employee was climbing a ladder when it slid out from under him, causing him to fall. Ladder was set up on snow and ice. Diagnosed with left hip pain.

This morning 1/20/09 the employee informed us that yesterday he was lifting a bottle of Compress oxygen. This morning he told us of the accident. We put a heat pack on his neck & sent him to Concentra Medical. Concentra said he strained his neck and they are going to do 3 or 4 sessions of physical therapy on him.
On 1-19-09 Employee was helping hang a miner cable when he felt a pain in his shoulder. On 1-26-09 the employee started missing work to have surgery.
EE bumped shin on a boulder. Sought treatment later in the week due to swelling. Did not receive restrictions until 01/28/2009.
Employee was working in #1 kiln bricking. When he climbed down from the brick rig he twisted his right knee on something, he's not sure what it was.
Employee was taking a golf cart down to position it for an Examiner. As he went around the corner at the bottom, he lost control of the golf cart and struck the metal wing wall, causing him to jerk his shoulder. He has worked until 06/17/2009 and has been taken off work for shoulder surgery.

Employee stated he was helping move the Cont. miner cable over out of the way when a piece of tie wire hung in an ear ring. Not realizing it was caught in the ear ring he continued to walk on moving the cable. It ripped his ear lobe into two pieces. Medical services glued it at first but then he had to go back and get stitches.
Employee was climbing off of powder truck when he stepped on frozen uneven ground causing him to fall on left ankle.
Initially employee injured his back on 1-16-09. He worked thru 1-23-09. He went to the doctor on 1-23-09 and started missing work on 1-26-09. He strained his back when he bent over to pick up cable cross-over pads.
The injured was putting on his boot in the bathhouse when he felt pain in his right side. The cause was later determined to be genetic weak abdominal tissue which allowed this condition. This is being classified as an occupational illness and not an injury.
The employee was greasing a conveyor grease line when a rock bounced off and struck the employee in the face knocking out a tooth and loosening two others.
The employee was changing a boom lift hose. A wrench slipped smashing the employee's finger.

Employee was walking to rail runner when he lost his balance and fell twisting his ankle.
Employee tightened new traction tires with ratchet on 1/15/09 and he states that his back started to bother him on the evening of 1/16/09. Employee reported this to the employer on 1/19/09 and sought treatment on that date. His first missed day of work was 1/19/09.
Employee was working in a fuse box and did not properly shut off the power to the fuse box first. Caused an arc that caused flash burns to employees hands.
Employee stated he was tramming a shuttle car in the #2 heading across bridges installed to maintain the roadway, While doing so he stated the canopy of the shuttle car struck the top causing a piece of rock or metal to strike his right eye.
Dropping miner cable that was hung across entry and pulled back muscle.
Cold weather made the pipe more brittle than normal. Typically the pipe can be cut to a certain point then the pipe can be broken with the equipment to finish the break. Under normal conditions the pipe could have been broken this way. Due to the cold weather making the pipe brittle the amount that was cut was not strong enough to keep from breaking on its own.
Employee stated that on 1/15/09 he picked up a bottom belt roller & carried it approx. 20' & threw it against the rib. At this time he felt a ""pop"" in his left shoulder. This incedent was under investigation & upon the diagnosis from an MRI on 1/27/09 it was determined that this employee has a torn rotator cuff & is being reported at this time as a result of the diagnosis
EE was using a sledge hammer to chip frozen material from a mud bin. EE stated that he had lower back pain at the end of his shift.
While attempting to free a frozen conveyor belt on a highwall miner car, employee pinched middle and ring fingers of right hand when he struck the belt with a 2"" x 2"" x 24"" long piece of square tubing he was using as a hammer. Middle finger was bruised and ring finger was fractured.
Employee was riding on a rubber tire mantrip, his SCSR was hanging over the side of the mantrip and when he reached down to pull the SCSR back in the mantrip, his hand got caught between the coal rib and the SCSR.
This accident was not reported to supervisor until 1/22/09. Employee was operating a 740 haul truck. He complained of hitting a rut in road and was thrown around in cab. Seat belt was on. When it was reported to supervisor, employee complained of blood in urine. Then when he went to Dr. he complained of blood in stool.
EE stated he was feeling ill and decieded to get some fresh air and when he opened the door on the dozer he passed out and fell off the dozer causing him to recieve several cracked ribs.
EE and witness report a very low speed impact between the two haul trucks. At time of incident both involved operators denied any injury. The EE was in a fully padded suspension seat at the time of the impact. There was minimal damage to the haul trucks. At a much later date, the EE claimed a back injury. The EE was denied worker compensation on two occasions for this claim.
Was walking from breakroom to shop when he slipped on ice. He was diagnosed with a hernia on 1/19/09
Employee was jacking a clutch on the propel using a port-a-power, the ram kicked out and hit him in the R.H. leg.
While walking on snow/ ice covered ground, the employee lost traction and fell onto his right knee. The temps were at or below zero for several days, and material had been spread in other areas, around the plant, to assist in traction.
Injured and co-worker were removing the top layer of belt rollers from a loaded flat. Roller twisted catching the left pinky finger of injured. Caused lacerations to the left pinky finger. Injured was transported to the hospital where he was treated for the laceration.

The employee reported on January 19 that he had neck pain not bad, sometime in December and again on January 14. He did not miss work. then On February 9 he called to say he was off due to his neck injury.
Two steel plates approx. 2'x 4' were set against each other upright near the base of the crusher, when EE was putting a clevis on one plate, they fell over his leg. EE's leg was broken from the weight of the plates.
While loading bolter supplies onto scoop bucket the employee twisted his back.
Employee reporting for duty collasped and fell down 7 stairs bumping head on grating. Employee was unconscious for 5-10 minutes. Medical assistance was administered and employee transported to medical facility. Cause of incident was determined to be personal medical condition and not a factor of the workplace.
While lowering his basket in the bathhouse, got something in his left eye.
As employee was working on regulator, adjusting panels off a ladder, a timber gave way causing the employee to fall and force of air and height roll him onto the ground hitting his torso and scraping his head. His hands were also injured on the sharp edges of the panels.
Injured was helping on a project to fix an electrical problem and needed the man lift to reach. The area he drove equipment down was loose dirt and rather steep. He turned it too tight and lost traction thus sliding and making an abrupt stop on a dirt bank up against the gravity take up guard, throwing him around inside the basket.
While making a belt move, the crew was positioning the tailpiece and applying tension with the scoop to align the belt. A chain broke that was hooking the scoop to the tailpiece, which allowed the tailpiece to slide around and strik the employees foot, resulting in a fracture to his foot.
Employee hanging miner cable in 3 heading out by right corner, a rock fell out between the roof bolt and rib, striking him on the left foot. X-rays were positive for fracture on left foot.
Bolter operator was drilling a hole for a roof bolt when a piece of rock fell out striking him on the upper right leg.
Ee was walking across bridge. The board broke causing him to fall and hurt his knee. No major problem. He returned to work after missing 8 work days.
The employee was filling the fork lift propane fuel tank. Upon completion of filling the tank, the employee closed the valve on the bulk tank, closed the valve on the fork lift fuel tank, he did not bleed the fill hose and when he disconnected the hose he was sprayed on left upper arm with liquid propane, causing a cold thermal burn.
EE tripped over a cable on the shop floor. EE was treated at the ER for a back strain and was released to return to work with no restrictions. On 2/2/09 EE went back to the Dr. and was treated for a herniated disk in his lower back.
Employee was watching the cable while the roof bolter was being trammed when the cable hanger broke causing the cable to sweep employee off his feet. He fell onto his side and landed on his rescuer.
The employee was walking down the wide side of the beltline when he stepped in a hole and felt a pop in his right knee. The employee has a blown ACL.
Injury was working on dryer line 3 with other empoyee. After lunch. He went back up the stairs and started across the elevated walkway. As he crossed the walkway, the grating shifted and he fell approx. 10' to the concrete floor below.
Employee was getting roof bolt off of machine when cable came loose from tie wire on roof and cable struck employee. He has soreness in shoulder blade area.
EE was riding in travelway on mantrip when the spare tire, which was on top front of mantrip, rolled out and slid. This trapped EE's left foot between the tire and the roof.
The person was closing a knife and cut his leg with the blade.
While cleaning under the belt tailpiece, employee was using a slate bar to dislodge compacted coal and hit his thumb between the slate bar and the tailpiece.
Empl.was using grinder to clean pin from the brakes of a quarry machine. His right index finger was pulled into the wire wheel causing an abrasion. The outer layer of skin was removed for 2/3 the dia. of the finger from the tip to primary knuckle. The abrasion was approx 2.5 in. After first aid, the empl.refused medical treatment,returned to work. No gloves or holding tool.
Injured was tramming a scoop in # 5 entry & a rail came into the deck and caught his cover alls and struck him in the back causing a fracture to his shoulder blade.
Alleged accident: Employee stated that he went to the bathhouse to retrieve something and slipped in a puddle of water, injuring his left knee.
The roof bolt operator was drilling the outside hole, on the curtain side, when a loose coal rib, turned over striking employee on the lower back and left hip.
At the beginning of the shift roads were being treated for ice. EE slipped on the ice, falling on his back side. He thought he was fine but as day went on he became sore. That is when he went to be checked out by a physician.
Injured person was changing pump barrel for unit 115B pump with two other workers. While moving parts into place to install, person felt pain in lower back and down left leg. Person began to miss work on 9/16/09 for surgery to back.
Employee was walking up the incline to notify the shop that he blew a hydraulic hose. He felt a pop in his left ankle, leg area. Employee was taken to the doctor for evaluation and x-rays showed a hairline fracture. Employee was released to restricted duty.
Opening door on D11R dozer, the door was frozen, the handle broke and he fell to the ground. He hurt his elbow, left ribs and lower back.
Employee was removing forms from the inner wall of the raw coal tunnel. While making a move to the next panel he was relocating his positioning hook at the same time the loose panel fell causing him to loose his balance and fall from a distance of approximately 6 feet.

The man was knocking a used drill bit off of a drill steel when the bit bounced off the canopy and down his shirt collar.
The DL operator, was removing frozen material on the DL shoe. He used a weed burner to heat the metal. He placed it in an access hole, then placed the cover back leaving hole partially open. Then operator strated digging. The oiler was asked to check on the weed burner. The operator heard an explosion. He found his oiler lying on the top of the shoe & started emergency care.
Employee was struck by a rock brow on his shoulder and back when it fell out, driving him to the ground. The rock measured 5' x 28"" x 16"".
Employee was climbing down from hauler and caught his foot between the last step and bumper, then fell, twisting his ankle and knee.
Victim was dropping cables for a power move when cable struck him on right hand.
Employee stated he was bolting and was straightening a 6' bolt for installation when it slipped in the grip of his hand. The bolt spun around causing the employee to jerk and fall foward injuring his shoulder.Employee started missing work on 5/7/09 by 220091320026
The employee was replacing vee-belts on the M-5 conveyor. While attempting to roll the vee-belts on, the torque arm moved out of its bracket, causing the employee to lose his footing. He then fell from his position, striking his groin area on either the torque arm hook or torque arm bracket. He fell approximately 8 feet landing in some loose material on the ground.
Miner operator was taking 2nd cut out of #5 left entry break when a methane ignition occurred. The ignition came back across the miner burning the operator.
Employee was in the process of reclaiming belt structure and was loading the structure in a scoop bucket. While doing so, he accidentally placed his left foot underneath the bucket as it was being lowered to the floor. As the bucket came down, it pressed down on top of the foot and fractured the tarsal bone. NOTE: He was wearing metatarsal boots.
Employee was helping a mechanic stand up a barrel and got his finger caught between two barrels and fractured his finger.
While grinding metal using a handheld electric grinder the employee got a fragment of metal in his eye. He originally thought it may have been just dirt until the eye continued to bother him for a few days. He went to the ER on 1/12/09 and it was found to be a metal fragment.
The employee stated that he felt a pain in his back while attempting to set a dozer blade wing into position to weld.
Injured was removing packing from a conveyor motor lead with a pry-bar and struck his hand with a small sledge hammer in the process. His hand swelled and he continued to work. He sought medical attention on 1/13/09 and was removed from work on 1/20/09 because the swelling was not going down.
Employee was assisting in building a sloping in the North Mains Crosscut 2 to 3 entry. The center of cinder block stopping fell over a solid block & hit employee on right foot. Fracture to top of right foot.
The tech was traveling down a ladder, lost balance, slipped, and fell approximately 6ft. The left heel hit the frozen ground causing a fracture to the calcaneous bone.
Employee was changing a roller on a conveyor. He was havnig trouble getting a nut off of a bolt which held the roller. When the nut cam loose, the roller came loose and employee tried to hold it from falling.
While digging a ditch for an electrical conduit, employee felt pain in left leg.
Walking from picking truck to job ee-slipped on snow covered ground-fell to ground.
Employee was walking down a stairway. When he got to the bottom, he tripped on a rug that was at the bottom.
Employee was taking supplies off the roofbolter to start repairs. While reloading the roofbolter, the Employee felt a pain in his back.
While getting out of the loader injury slipped on the ice and pulled something in his knee.
Employee was operating miner, moving from #6 to #8 entry. Twisted his left knee while crawling.
Screen - Change - Cutting Crown Bar Rubber - Knife slipped and cut right arm.
After punching out for the day, employee was descending down from the stairway when he lost his footing from the middle landing of the steps and tumbled down the remaining 9 steps.
EE set CM over to right side of #4 right, head hit rib something popped off head and struck right eye, causing injury.
Employee finished servicing truck and went to park it and fell from step approximately 4 foot to the ground on his side.
Employee was using a slate bar to close the door on a railroad car when the slate bar broke about ten inches from the end. It struck him on the right lower leg causing a fracture to the tibia and fibula.
EE had cranked up the lights to turn them on. When he went to start them, the fiber clutch failed. The lights started to come down with the crank handle spinning. Ron tried to stop the handle and was hit in the left hand, on the ring finger.
The miner was walking under the primary crushed stone plant when an approximately 3"" stone fell from the conveyor belt and hit his hardhat and took a part of the neck area. The miner was immediately taken to the hospital where medical attention was provided. The tests showed a bruise on the neck area. The miner was sent home to rest.
Employee was taking down 2"" pipe on utility truck. He was holding the other end of the pipe with one hand while he took the coupler off. When the coupler came off the far end of the pipe slipped out of the chain and fell on the back rail of the work deck. The pipe bounced up and struck him on the left side under the rib cage.
Closing doors of railroad cars with a prybar and slipped and fell on some ice.

EE was walking to work on the road west of tunnel, when he slipped in the mud, falling on his right hip. He continued to work and perfomed his job duties that day.

EE was in the process of giving his pre-shift to the fueler, when he fell to the ground from his dozer. He received 3 fractured ribs and a laceration to the right side of his head, requiring 2 staples.
EE was using a come-a-long to pull together some belt takeup part when he felt pain in right groin area. EE continued to work until 1/14/09 at regular job tasks.
Employee initially injured his right elbow while using a chain ratchet to splice a belt when he struck a mine water line. He worked until 01-13-09 with only soreness. At that time his arm set up infection and had to seek medical attention to have the infection removed.
Employee was exiting a 930 loader, when he lost his footing and fell off of the steps to the ground, landing on his right knee and bending it back under his body.

EE states he lifted a tailgate weighing approximately 40-50 lbs and put it in the back of a pickup truck. He felt a slight strain at the time, but didn't think he had injured himself. The next day he required medical treatment due to back pain.
A catwalk had been damaged by coal truck and was barricaded off with metal straps on both ends to prevent entrances on either end. He went over straps, grabbed handrail, it broke and he fell backwards and hit his head on main belt structure.
On 1/7/09 employee was painting inside area known as TP; on 1/8/09 he began coughing up blood; on 1/9/09 he was taken to Limestone Medical Center where he was diagnosed with chemical burns & pneumonia. That facility was not able to treat & he was taken to Hillcrest Baptist Hosp., Waco, TX. Employee is a smoker.
Employee complained of his back hurting on 1-7-09 and started missing work on 1-14-09. Doctor's report shows that the employee is suffering from degenerative facet disease.
Scoop oper. plaster stopping-height 8' elevated bucket plastering top part of stopping, stepped down and got between bucket and stopping. Had back towards scoop bucket, leaked down onto back of scoop operator, scoop operator said he he was ok. Operator checked and brakes worked.
While replacing an ejector cylinder on a ram car and lifting it into the bed felt pain in his lower back. He felt like he had strained his back but did not seek any medical attention. Continued working until placed on a 'no-work' order by his personal physician on 4/7/2009.
Employee set lunch on skid on ground, raised up and bumped into roof bolt. Loose debris and one 6"" flat coal piece rolled down back.
EE was operating a bagging machine, twisted wrong and felt discomfort in his lower back. He continued to work for a number of days and began taking time off on 01/21/2009.

Employee was trying to turn over a very heavy chain, when he felt a ""pop"" in his back. This was dismissed as a minor pulled muscle. When trying to walk up a ladder to the dragline, his legs gave out, resulting in a fall from the ladder.
While utilizing a come-a-long to free a stuck conveyor flight in the stageloader, the hook slipped out of the flight allowing the chain of the come-a-long to strike the employee in the face.
Ee was walking in front of the maintenance shop and slipped on snow covered ice. It was very bad weather that day as it was snowing very heavy. The x-ray was negative, but was diagnosed with an ankle sprain.

While operating shuttle car, operator felt pain in right arm after shuttle car hit a hole in roadway. Operator continued to work until 1/30/09 when he sought medical treatment for the pain. Pain was diagnosed as a repetitive strain injury. The operator uses his right arm to operate the steering on the shuttle car. First day off the job 2/2/09.
Employee was trying to move a rock on the flipper table. Started to fall and put his and on flipper table and twisted his left wrist.
The employee stated he was bolting the rib when a piece of coal 24"" x 24"" x 2"" thick fell and struck him on the shoulder. The employee did not go to the doctor until 1/7/09 when he started missing work.
The employee was picking up a concrete block from the ground and felt a sharp pain in his lower back. The employee strained a disc in his back and will receive surgery to correct the condition.
The injured miner was installing roof bolts, when a piece of draw rock measuring 6"" x 2"" x 3' long fell, striking miner in lower back.
While clearing frozen material off of a side wall of a bin, a large section of frozen material slid, striking the employee in the left leg/ knee. The employee went to the local E.R. and was diagnosed with a hyperextension of the knee.
After parking the Fuel/Lube Truck employee walked behind the truck where he slipped/tripped on the frozen ground resulting in a sprained ankle.
Employee exited a control room to a platform and while descending steps caught the back of his boot heel on the previous step and fell backwards injuring his left knee.
Contractor was dismounting Cat D-11 dozer. As he got down to the lift arm, he slipped, fell backwards, struck the ground, injuring back and right wrist, sustaining a fractured thoracic vertebrae and right wrist.

Employee stated that he was driving a Ramcar and ran over a 4-5"" timber causing him to bounce up and hit his head on the canopy. He continued to work and then was taken off work on 01/13/2009 with a back injury. Employee has had two prior surgeries and a Doctors report showed degenerative disc disease and had back problems before this incident.
EE was operating a 1500 Komatsu Haul truck being loaded by an O&K 340 shovel and stated he received a jar from a rock that was being loaded. EE continued to work until 01/16/09 when he went to Dr. and was placed off work with a strain to the back.
Employee stated while walking he tripped and fell into air pump.
The employee was verifiying the process equipment, suddenly a release of florida lime blew in his face and the left eye.
The employee was operating a s/c and running the haulroad bouncing out of his seat contacting the canopy fixture with his head. The incident resulted in an injury to his neck, sprain, and the employee is losing work time.

EE was taking 4 waterline apart when it busted and projectiles from waterline stuck EE on his right hand. EE finished shift and reported to work on 1/6/09 but did not finish shift. EE is scheduled by doctor not to return to work until 1/16/2009. X-rays were negative.

Employee was carring oil buckets on fueling platform when he slipped and fell striking his left side on the handrail.
Employee was hooking chain up to tool car and scoop to pull tool car closed to section. He told the scoop operator to lower the scoop bucket. The operator dropped the bucket and injured screamed that the bucket was on his foot.
