Accident type, without injuries

A roof fall measuring 70' L x 20' W x 8' H fell on break 35 of 5 belt track entry. The area had been dangered off, cribbed and will not be cleaned up.

Rock fall occurred in the 1 North Mains in the #3 entry at break 40. Fall measured approximately 20' wide x 50'-70' long x 3'-5' thick.

Rodeo hoist is operated from the Meikle hoist control room. There was a loss of communication to operate the hoist safely. A new communication relay was installed, the hoist was then run several series of tests. The hoist was brought back up up on line and fuctional at 3:00 PM 06/01/2009.

While making a pre-shift examination of the mine the examiner discovered water coming from the face area of the retreat mining section. The water was pooling in an area that was blocking the main intake escapeway from the working section. This was reported to the agencies as a mine inundation by water.

When checked, it was discovered that the Cooks Branch Elevator would not operate properly. After further investigation it was discovered that the door button had a short. The elevator is not used to transport men and probably does not fall in the category of reportability.

A new divider, carried on work deck to where it would be installed, came into contact with another divider. Resulting damage to work deck and its disconnection from the skip. Deck wedged in the shaft and the miners were taken to the 4-Level of the mine. All miners were evacuated from the mine until the work decks could be retrieved and all systems returned to normal.

A roof fall measuring approx 20' X 30' and approx 10' high. This was an intersection in a bow tie in F1-butt in the #1 room on the intake side of F1-butt.

No Injuries occured when the hydraulic line broke below Clinker Cooler Bedgrate causing oil to be picked up by the cooling fan air flow and taken into the chamber with hot clinker. The oil mixed with the high temperatures caused an ignition.

An unplanned roof fall occurred in the #2 gas mine at spad # 1355 on the belt entry that measured 58' long x 19' wide x 14' high. No injuries reported. The area was developed on 2-1-09.

A fall of the roof occurred on the 1st NE submains at x-cut # 11 on 4B supply road. Approx. 50 ft long, 18' to 18.5' wide and 6' to 8' thick. Ventilation, personnel nor equipment were involved or affected.

An unplanned roof fall occurred in the Stockton mine that measured approximately 30' long x 18' wide x 4.5' high. No ventilation was affected. No injuries were reported. The area was developed on 8-30-94. Occurred at brk. 191 spad # 1434.

Rock fall occurred in the 3 East Mains in the #2 return entry at break 46 from #2 entry, it fell 20' in 2 right and 40' into 3 right. Fall measured approximately 60'-80' long x 4'-6' thick.

Due to mistaken procedure by electrician following bottom switch replacement on Koepe access cage, the PC and lilly controller got out of ""sync"" causing malfunction. Mine evacuated via prod. shaft. No injuries/no problems. Koepe operational at 10:45am. Forty-three persons in mine at time.

A roof fall occurred in the East Mains in the intersection of crosscut 7 in the #2 entry measuring approximately 25ft. long x 18ft. wide x 9ft. high. This area was supported by square cribs, 4ft. fully grouted roof bolts and 8ft. cable bolts.

A fall measuring 25' long, 30' wide, and 8' high was discovered 3500' outby the working place during a routine exam. The fall was located in the return entry of the 001 section 25' left of spad 58814. Affected area was developed Dec 07 with 5' torque tension roof bolts as the primary roof support. Fall occurred due to laminated coal and rock above the anchorage zone.

Fuse tripped the main hoist and caused the hoist to be non operational until approximately 3:00a.m. on 05/27/09 when the fuse was repaired and the hoist was placed back into service. This was electrical damage not mechanical damage.

A rock fall occured in the 1st Northweast main. #1 entry from 34 + 26 to 3856, #1 entry from 40+36 to 41+51, #1 entry 43+53 to 45+66, #2 entry from 39+16 to 39+86.

During an inspection a fall was discovered along the return airway on the Tom's Branch section. The fall was located between spad 600 and 605 in an intersection. Fall measured approximately 20' W x 20' L x 10' H. The fall was discovered by MSHA & Mine Management.

A roof fall occurred in the neutral entry of the main north. No injures or disruptions to ventilation. The fall was approx. 40' long by 8' high by 18' wide. The fall was discovered approx. 2:32 a.m. Fall was at spad 27 & 84 xc39 No. 7 entry of the main north.

The brake tips on the Powhattan No. 6 mine # 2 portal elevator were dirty and caused elevator to lock up. The brake tips were cleaned and the man hoist/elevator was put back into service. The elevator was down for approximately thirty two minutes.

A roof fall outby 02 working section was discovered by shift foreman at # 1 Entry, 1 block inby spad 5895. Area was reported and cribbed and dangered off.

An unplanned roof fall occurred in the #2 Gas mine at spad # 850 in the #7 entry that measured 25' long x 19' wide x 12-14' high. There were no injuries reported. The area was developed on 6-30-08.

A roof fall was found on the South Mains ""A"" mainline belt inby 51 cross cut. Fall approx. 50 feet long, by 15 feet wide by 2 to 12 feet high. Area was bolted with Install II bolts and resin assisted. The supplemental support consisted of bacon straps, wire mesh screen,wood cribs and wooden beams. Cause of this fall was weathering of the roof.

Roof fall occured in a crosscut of a clay vein fault area intersections where cable bolted. The fall occured in a worked out section of the mine where no work is being done.

A roof fall was discovered in the 2 North 7 Submain, #4 Entry at Crosscut 13-14. The fall measures approx 20' long X 16' wide X 6' thick. The area is supported with 5' torque tension rebar bolts.

Oil leaked out of the HGG due to pooled oil in HGG combustion chamber caused by pumping oil into the HGG, the oil not burning and extended pumping time. **Excessive oil had pooled at the Hot Gas Generator, while at the same time the pilot failed to ignite.**

Electrical storms in the area created a power surge causing two (2) fuses on the B phase of the thyristor for hoist #4703 to fault.

Knight Hawk Coal LLC, Royal Falcon Mine has had an unplanned roof fall in #4 entry at the +570' cross cut in the Main North return. No equipment or people were envolved in the fall. The fall was called in within 10 min. of discovering it. All the required steps have been taken.

On 5/22/09, during his weekly exam, Employee found a roof fall in #2 entry between 41cc and 42cc on abandoned section. Fall was approx. 60' long x 20' wide x 4 1/2' thick. This fall did not interfere with ventilation to working section.

At 8:30 am a squirrel crossed between two of the main electrical wires (13,800 volt) from Avista's substation to our substation. The arc knocked out all the power to the mine, shutting down the hoist. All work underground was suspended until Avista was able to restore power, approximately 3 hours.

The fire-boss discovered a roof fall in the escapeway while making his rounds. Two seperated adjacent escapeways exist and the lifelin and escapeway was routed around the fall. The fall area was timbered and breakered off to prevent travel from all sides. The fall did not occur above the anchorage zone of the bolts.

On Thursday (5-21-09) at 8:25 the elevator stopped due to moisture accumulation in the proximity switch. MSHA and WVOMHST were notified timely.

An unplaned roof fall was discovered by the mine examiner approx. 650' out by the 001 section. No people or machinery are affected. The fall is 20' wide x 20' long x 6' high. The fall is located in the #7 entry inby survey station 454.

A rock fall was discovered on 5/21/09 by a representative of the company and MSHA inspector on an old panel, 1A panel. The rock fall has been timbered off and dangered off. This is a worked out area.

An unintentional roof fall occurred on Unit #3 at Entry #7 at 18+60. The fall occurred due to a slip running through the intersection. The fall was originally supported with 5 and 6 ft. conventional bolts. The fall is approximately 30 ft. long, 20 ft. wide and 5-7 ft. high. The fall will be cleaned up and resupported.

IGNITION: The Continuous Miner was cutting coal and bottom rock in No. 3 Entry on No. 5 Section on the right side of the entr when an ignition occurred on the Mine Floor. The ignition lasted approximately 10 seconds and was put out with the water hose.

5-North escape way hoist was out of service at 8:30 AM due to a broken control handle. The hoist truck was brought to site and set-up and tested. At 12:10 the EE returned to work.

IGNITION: The Continuous Miner was cutting coal and rock in the No. 4 Entry on the No. 5 Section on right side of the entry when an ignition occurred on the mine floor. The ignition/flame was put out with one fire extinguisher.

This area will be rehabiliated all entrances have been breaker off. This fall was phoned in the clark blackern on 5/20/09 filed superviosr.

During a routine inspection the governor for the #6 elevator was found to be defective. The elevtor was down from 12:55 p.m. to replace the governor.

A rock fall was found approx. 12:40 pm in the #1 main return, in #1 entry between brk. 48-49 in the secondary escapeway, the fall was approx. 100' long & 8' thick, the area has been dangered & timbered off, the secondary escapeway has been re-routed to the #3 entry, #4 return, brk. 3. A meeting was held with all employee's on changed with the secondary escapeway.

A roof fall occurred in the #11 seam, 2 East panel at crosscut 62 at intersection of #3 entry. The fall measured approx. 22'x32'x6'-8'. The area was supported with 5' fully grouted bolts.

There was an issue with a breaker on the ""B drive"" on #3 hoist. Electrician changed the breaker and the hoist was back in service at 12:10 pm. There were still two escapeways throughtout the incident.

A roof fall occurred in an area that had been cribbed and dangered off in #7 entry, cross cut #25 on #2 unit (MMU 006-0). The area was roof bolted with 6' and 10' bolts. The fall appeared to be fractured stack rock. The fall will not be cleaned up.

Management elected to temporarily discontinue the normal use of the Camp Run elevator to investigate a fault condition with the elevator. Schindler repair service was called to investigate. The fault was eliminated with the replacement of a door switch. The elevator was returned to service at 3:34 AM. No persons were entrapped nor was their any damage to the elevator.

An unplanned roof fall occurred in #2 Gas Mine that measured approx. 19' wide x 19' long x 12 to 13' high. This is a continutation of a fall that occurred on 9-6-08. No injuries reported. Area was developed on 3-25-08.

An unplanned roof fall has occurred in the first left off NO.1 Mains, the fall is located two crosscuts inby survey spad no. 541 the fall measures 40' long, 20' wide, and 6' thick ventilation and travel are not affected in this area.

#3 hoist kicked out on drive converter a fault. Electrician was called out of the mine to check and reset the hoist. Hoist was back in service at 8:30 am. there were a still two escapeways throught out this incident.

Men were bolting this area 2nd East 1 Entry 1 to 5 XC. They noticed truss bolts taking unusual loads. At this time they removed equipment and men. After the need for longer cable bolts was determined operations at this location stopped. While materials were being obtained, the roof fell. The roof consisted of deteriorating mudstone and shale layers up to about 10'.

Examiner found a roof fall on the 3rd North belt just outby x-cut 93 at approx. 1:30 pm. Operator did not feel this roof fall was at or above the anchorage zone but was reporatable for impeding travel in the belt line. MSHA was immediately notified and roof fall was investigated. The roof fall was approximately 20 ft wide, 35 ft long and approx. 4.5 feet in height.

A roof fall occurred on #2 unit, #5 entry, 8 crosscut. The fall measures approx 20' long, 16' wide and 12' thick. The area is supported with 5' torque tension bolts and 12' cable bolts.

Hyd. hose blew spraying engine components, igniting oil workers unable to extinguish before it spread on to flammable components in cab and around engine compartment.

An unplanned roof fall was discovered on weekly exam app. 12000 ft from the active #1 sect 1 xcut inby s.s. 2897 in the right return the fall does not impede travel nor block airway. The air is 100 ft in length x 19 ft x 6.5 in height. all entradces are areakered off. It will not be rehabiliated this was phoned in the field supervisor on 5/15/09.

Roof fall has occurred on the #2 section ( MMU 002-0) The fall was 42 inches thick, 17 feet wide and 22 feet long in the #4 right crosscut at survey station #853.(Narrative per Violation No. 8070199)

The mine encountered an unintentional roof fall on 1 WA, XC #29, between entries #4 and 5. The fall measured 20'L X 18'W X 4' thick. The area was developed 3/1997, supported with 4' glue bolts with straps. This fall will not be cleaned up.

Lost power @ 3:18 a.m. and fan. All employees were evacuated out of mine. Ventilation was restored @ 4:00 a.m. Employees did not go back in the mine due to rising water in the pit. At 5:08 a.m. the water breached the return portal and went to xc-7 MW return and over to xc-6 MW travelway. Reported at 5:19 a.m. There was no k order issued.

A roof fall was found at 10:30 am in the 2nd East Sub-Main, #5 entry, 30+10, during a rock dust survey being taken by MSHA. The fall was 40'X19'x7' and had been mined in March 2009. The area had been supported with 6' and 8' roof bolts and was caused by an undetected slip in the mine roof. Ventilation or passage of men was not affected.

At approximately 4:25 am the Hart street elevator failed to operate. The investigation revealed that a limit switch located in the head house had water in it, which caused it to fail. The source of the water was believed to be condensation. The limit switch was replaced and the elevator was returned to service at around 6:00 am.

An unintertional roof fall occurred 5,000 outby. The section, one break inby spad No. 3433 in the return entry. The fall measured 6' high, 40' long and 20' wide. The fall does not affect the travel of men or the passage of air.

A roof fall occurred in the belt entry of the 5th Panel North. The fall covered the 2A belt tail piece. No injuries or disruptions to ventilation. The fall was approx. 50' long by 6' high by 19.5' wide. The fall was discovered by a repairman at about 5:30PM. Fall was a Spad 5+04 (XC 12) in No. 6 entry of 5th panel north off 1st main east.

A roof fall was discovered outby spad 25737 adjacent to 11 belt line. Estimated size of the roof fall 10 ft high, 19 ft wide and 140 ft long. No injuries were reported as a result of the roof fall.

A roof fall occurred on # 2 unit, # 3 entry, # 6 Crosscut. The fall occurred as 12' cable bolts have penetrated the water table. The fall measures approximately 25 feet long, 17 feet wide and 8 - 10 feet thick. The area is supported with 5 foot torque tension rebar and 12 foot cable bolts.

Due to exesive rain fall in a short period of time, a mud slide occured at no 1 portal, partially blocking Left return.

Due to abnormal amounts of rainfall an inintentional inundation occurred causing water to collect from crosscut #2 to crosscut #11, in all entries, on the South Mains, resulting in the inability of 7 men to exit the mine.

An event of May 9 of 6"" - 12"" of rain after 8 days of rain caused a failure of diversion lines that were designed according to SMRAC Act of 1977 allowing water to enter the mine.

After 9 days of rain & 5.75"" of rain on Saturday May 9, 2009 and upon further exploration of the mine. Water had entered the mine filling a low area and some of the face area. The water traveled by the face by penetrating some stoppings and traveled the low area of the mine. The mine was not working due to area flooding.

About 4 A.M. discovered fall entry #6 on 001 section, in return, 60' inby spad 2184. Fall was 55' X 20' X 5', there was no equipment involved, ventilation was not affected, passage of people was not affected. Area was breakered off, and we have no plans to rehab this area. Both MSHA and KY OMSL was notified.

Due to excessive rain fall, the drains near the mine portals overflowed and allowed water to flow into the mine. No one was injured or trapped. We ditched the water away from the protals and pumped water from the mine. All the employees were withdrawn from the mine immediately.

An unplanned roof fall occurred in the 5-Block mine that measured approximately 20' long x 10' wide x 6' high. The fall fell behind the continuous miner, cutting the cable. The miner was in the #4 entry, spad # 189. No injuries were reported. The area was developed 5-7-09.

A roof fall has occurred 1 crosscut past spad #117 in the return airway, half way between #7 and #9 entries. The fall measured 60' long, 7' high and 20' wide. Persons, ventilation nor equipment were not involved. This area will not be rehabilitated.

Failure of drive communication board in hoist system resulted in shutting down the hoist to replace the board. It is believed that a thunderstorm from the prior day may have contributed to the failure of the drive communication board. Hoist was out of service for approximately one hour.

There were 2 elevator service employees who became stuck on the Livingston Portal elevator. This occurred when they stopped to examine the governor rope and were going to continue up the shaft. There were no injuries or equipment damaged as a result of this incident.

A roof fall was found by examiners along the 3 West roadway at crosscut 36. The fall was located at the edge of the roadway between the belt and the roadway. The fall is between entries 4 aned 5.

On 5/8/09 at 440 am When I arrived at the mine site. The mine fan was not operating. After investigating found that one of the six blades for the fairchild fan broke off putting the fan out of balance and throwing the v belts off the pulley. Replaced the 50 hp Fairchild fan with a 50 hp Ingersoll Rand Fan.

A roof fall occured at 10:45 pm in D-Butt at #35 intersection of #4 entry, spad #8369. The fall was 30 ft long, 20 ft wide, and 4 ft high. The area was bolted with 42"" bolts.

The fall occurred at the 3 Mains Section (51 Crosscut, #7 Heading Intersection) at 2000ft. outby the face. The area was bolted with 6ft. fully grouted resin bolts and 12ft. pt cable bolts. Ventilation was not disturbed, and travelways were not affected. No equipment was involved, and no injuries were reported.

A rock fall was found and reported to to MSHA. The rock fall or roof fall occured on the mmu 001-0 section. The roof fall is in the n% 5 entry 1 brk. Inby survey station 4143. The roof fall is at brk n%9 in the intersection. Roof fall is approx. 6'-7' thick, 16'-18'long and from 3' to 7' thick. Fall dangered and timbered or cribbed

A roof fall was discovered at 8:47 am in the 2nd East Mains, x/c #4, #1 entry. The fall measured approximately 20' long, 25' wide and 7 - 9 feet high. The area was bolted with 5' fully grouted bolts. The fall did not impede ventilation or escapeways.

An unitentional roof fall occurred in the belt line of the 11 I longwall section. The fall occurred just outby the #12 x-cut. The fall measured approx. 55 ft long by 8-10 ft high by 16 ft wide. No injuries or equipment damage with this incident.

After performing regular scheduled servicing on #3 hoist, the hoist tripped on ""drive converter A"" fault upon startup. The hoist was checked out and reset and back in service at 10:35am. There were still two escapeways during the incident.

#2 Hoist went down due to the whisker switch sticking in ""on"" position. Whisker switch was repaired at 9 a.m. No mine employees were trapped; two means of escape were available.

A unitentional roof fall was found one break inby spad # 659 in # 2 entry. Fall runs from the intake section of # 2 entry thru the break towards # 1 entry on the northwest section. The fall is approx 8' high x 18' wide x approx 58' long.

Mine superintendent found a roof fall during is weekly examinations. Fall was at Bk 68 in # 1 entry. Fall was approx. 18' - 20' wide x 45' - 50' long x 5'to 6' high. Area was timbered and dangered off. Fall was on old inactive section.

Clay veins discovered initial mining. Area had ben supported with 4 ft bolts & resupported w/10' & 12' cable bolts. Coal Seam @ approx. 8'.

The slope hoist was taken out of service due to two kinks in the rope. 260 feet of rope was removed and the rope reattached to the slope car. The car was returned to service at 5:58 pm. No escapeways were affected and no individuals were on the car at the time.

At approx 0600 the chute coming off of E231 plugged. At some point it appears that the V-belts on C132 had burnt off & the belt had stopped allowing hot material to sit on the belt & build more heat. The material was able to get to C132 due to a rotted gate in the C231 to C132 discharge chute. The Fire was found and reported @ 0615 by Kiln Operator.

Mantrip slid into hole while employee riding in back, bounced him up, he landed on rescuer, pain left hip/lower back. Dr put him on physical therapy, didn't miss any work from accident, later sent to receive spinal injections, injection sites began to leak fluid, patches placed over holes,started missing 9-14-09. patches applied on 9-16-09. We feel this qualifies as an illness.

The employee was pushing dirt over a 20 ft. bank, and thought he had enough dirt in front of the dozer to drive off the embankment to a level below. There was not enough dirt in front of the dozer and it nose dived to the lower level, causing a whip lash effect to the employees neck.

While positioning a section of roof screen above the miner, the employee stated that the vent tube was dislodged, which fell striking the employee on the top of the hard hat. The employee suffered a neck strain.

He then installed a bolt in the next row and swung the drill head out to spot a bolt, when coal/rock rib measuring 14' L, 20"" T and H was 68"" struck members left leg, pinning his leg against the drill head near a panel switch. Left leg above knee received laceration & abrasion wound. Surgery repaired muscles and tendons.

After loosening the jamb bolts, and removing the pin that holds the pipe in place; the pipe slid through his hands and before he could stop the pipe and fixture from slamming down it smashed the tips of the two middle fingers on his left hand.

Employee reported: Backing continuous miner from cut. To keep the machine cable pulled away from the tram pads, he was using his right foot to guide the cable along the mine ribline. As the machine was backed and pressure applied, his right knee popped. The following day the knee began to swell & become painful to walk. Started missing scheduled work days Friday May 22, 2009.

The injured person was removing a piece of rock out of a Long-Airdox bridge conveyor. When he let the rock fall, he didn't move his right foot out of the way letting the rock fall on his right foot.

Left arm hung in the belt tail. Broken humorous bone.

Servicing ramcar. Slipped and fell while getting off. Hurt right foot.

Operator was pushing shot rock out of shot with D11 dozer. As he was backing up a rock rolled in behind him and he didn't see it. When he backed up on the rock the dozer slide off and jarred him and the machine and hurt his back.

Employee stated that he went to C-Belt Hole to get a solid steel bar to hang 1000 ft. of conveyor belt. While lifting the bar he felt pain in his lower back.

Employee was driving a golf cart along the underground conveyor beltline and his leg struck an emergency stop box that protrudes out from the beltline. Employee suffered a broken left leg.

While walking across the fourth floor level, employee hit a pipe support with his head, causing a cervical sprain and a headache.**Accident was not reported until May 27, 2009. Employee was seen at hospital ER on May 28, 2009. He missed May 29th & June 1st and returned June 2nd at full capacity.**

Employee was drilling an anchor hole with hand-held auger. The auger was bent and caused the employee's hand to be caught between drill and anchor pin, injuring his left middle finger.

Miner inhaled smoke from a power center that failed. This miner did not begin missing work until 5/29/2009.

Employee states he was walking around the side of the miner when he stepped on a slab of coal with his right foot which caused him to fall on his left foot. The fall resulted in a sprained left ankle and a spiral fracture to the left fibula.

Employee was exiting the cab of the vehicle she was driving that had been parked with the driver's side front and rear axles partially on a stem pile increasing the distance from the vehicle step and the ground. As the employee's left foot contacted the ground, her knee buckled. An MRI revealed a torn ACL. Surgery to repair the torn ACL took place on 07/16/2009.

Employee states that he had bolts made up and staged along the rib. The bolts slid off the rib and the corner of the pizza pan struck his left forearm causing a 1"" laceration.

A piece of rock fell on bolter canopy. When ee tried to remove the rock his right index finger was mashed between the canopy and the rock.

Injured was picking bottom and rib out under the continuous miner to free trailing cable. A small piece of slate flew from under the miner, becoming lodged in the injured's right hand (near index finger). Please note: the injury occurred on May 15, 2009, but the injured did not begin missing work until July 7, 2009.

This is an ""Alleged Injury"", this report may be amended upon further investigation. EE was changing bearing on conveyor, feeder was locked out, taged out, along with conveyor. EE was crawling out of feeder, when he took off hard hat and a rock flew from secondary feeder and hit him in the head.

Employee was attempting to ascend a ladder to the cab of a CAT 777 haul truck, when he slipped and fell on his back. Employee was in pain and tolerated it without reporting this original injury. Employee claims to have aggravated the injury on 6/11/09 while trying to bang a pin from a bucket of a CAT 365 excavator. The injury was not reported to the employer timely.

While performing his regular job duties as a roof bolter operator, employee reached up to get a section of drill steel when he got the middle finger of his left hand caught between the roof bolter pot and the mine roof.

EE stated that he felt a pain in his groin area and felt a knot swell as he was loading supplies on bolter. On May 22, 2009 he was diagnosed with a hernia that would require surgery.

Subject employee suffered a minor cut on his right hand index finger while mounting a welding trailer on to skid loader hitch to be move to other site for a maintenance routine.

Employee was putting up the outside bolt in the 2 left cross cut. As he swung his drill pot to install the inside bolt a piece of drawrock fell scraping his left leg and foot. He went to a specialist and was told to remain away from work. He was diagnosed with strained ligaments.

EE was hanging miner cable while on his knees, when the Fairchild 35-C scoop backed around the corner and ran upon his left metatarsal boot.

EE noticed rock jam in crusher. EE stopped crusher to remove rock. He picked up rock (about 30 lbs) and later felt back pain in lower back.

Employee was walking down steps in prep plant when he caught back of boot on step and decended 4 steps to the concrete floor. Employee sustained a simple undisplaced fracture of a bone in the left foot.

The employee was pinning a place when he went to put the pin in the roof he felt pain in his stomach. He continued to work until he heard from a specialist. The employee started missing woth on the 26th day of May, 2009. The employee had to have surgery for a hernia.

Four men were on the ground prying with a 16 ft bar trying to turn a 36 in crusher drive pulley. The prybar was placed behing a bolt and against the pulley. The bar slipped off the bolt and the bar moved toward injured and he started to fall backward. He rotated to catch on his hands and his right leg came across a bar laying on the ground.

Employee was mucking a slusher stope borehole when he was struck in the forehead by a rebar bolt. To the best of our knowledge, the LHD tire kicked the bolt up off the ground.

Pushing dirt off high wall, too close to edge and went over edge. Loader landed on its top, Loader operater had seat belt on.

Employee was pulling on a rock that was hung up in his carrier chain when he felt pain in his lower back.

The rub rail on the miner was being removed when it fell and hit Ee on foot. The impact broke his little toe. Required surgery to repair. Ee will be off work a couple or three weeks.

The air lance appeared to be plugged, so EE unhooked the air hose. (He was wearing safety goggles, but took them off. At this time, he only had on his safety glasses and sideshields).When he opened the valve to the lance, the air and copper sulfate crystals blew into his eyes.

Injured was tramming bolter into #4 Entry to bolt 4 right Crosscut. He moved from deck to ""inch"" tram controls. As machine pivoted around, a curtain board on machine caught on a rib and sprang towards operator, stiking him in the mouth. Busted upper lip and possible fracture of facial board.

EE was operating 988 G attempting to straighten up corner of sheet , loader slipped off large rock he was attempting to break loose from wall. The operator was shaken when the loader slipped off of the rock. He was wearing a seat belt.

Two mechanics were trouble shooting a breaker and plug on the transformer with a muti-meter. The breaker had lost power several times, when mechanic was checking the plug, with the meter, it went phase to phase.

EE was assisting installation of top rollers on new belt structure. While positioning roller, men on the other side of the belt pulled on the structure causing the top roller to drop into position. The 5th finger of his left hand was caught between the roller assembly and the top of the side rail. He jerked and pulled nail off finger, 3 stitches, started missing 5/12/09.

Was pulling miner cable out of roadway for miner to move across section, when he experienced pain in his lower back and hip. He went to E.R. at McDowell Hospital, was x-rayed and was told to see family doctor. Started missing work on 5/18/09.

Employee was swinging a hammer on the gypsum bin on the #3 finish mill feeder weight floor and he allegedly felt shoulder disconfort. This was considered a first aid case and he did not miss work and was on full duty. On 9-17-09 he had a medical procedure that requires this to be MSHA reportable. He was a member of a volunteer fire department during this entire time.

While walking along the beltline. Entry Injury stepped between two rocks and stumbled, in doing so he fell against the rib and accidently pulled rock from the rib when fell and struck him on the left ankle. He didn't report accident until 5/13/09 to his forearm.

Employee was walking longwall face while pulling shields and sliding hand along rail. When he caught the small finger of his right hand on a rail joint. He fractured small finger.

EE was sliding a top center belt roller across the top assembly frame at #86 wall, 3 North #3 Belt, when the roller fell and bounced off the bottom belt, catching his left little finger between the assembly frame and belt roller. EE was wearing Meta Miner Gloves. EE sustained a fracture to the left little finger.

EE was installing a 10' cablebolt using fast feed to push bolt to top, bolt bent dislodging a portion of roof. Dislodged rock and struck EE on left elbow, causing a small cut to elbow.

Walking from tail piece to feeder stepped over feeder timber support Lt foot slipped off belt bed rail twisting ankle severely to one side. Cause of injury slipping on material in travel way. Employee was getting on his dozer at the beginning of his shift, when he slipped and fell between the push arm and track. He injured his left shoulder and left hip.

EE stated he was at the river portal putting old rail in pile with other debris to be disposed of later. He was moving a 6' piece of 85# rail out of the track clearance space. He had moved 1 end of the piece out of the way & moved to the other end. As he picked it up he got it about waist high & the wet rail slipped out of his hands & landed on left foot resulting in a fracture.

EE was attempting to push a roof bolt up into the top by hand and felt something pop in lower back. EE was placed on long spine board, head was stabalized and EE transfered outside b/met, boss and bolting partner.

Employee was removing lodged lump of coal with slate bar. In an attempt to free the material, the slate bar & lump fell out of the bin area striking employee's right leg ultimately causing a scrape on the leg. Employee refused medical attention initially, but went to family doctor on 5/26/09 because of redness. Doctor put employee off work for rest of week for infection.

HE WAS CUTTING THE BANDS ON THE STRUCTURE PILE AND THE BAND BROKE CAUSING STRUCTURE TO ROLL ON ANKLE.

While throwing Miner Cable out of roadway for roof bolter to cross over, felt something give in lower back.

EMPLOYEE WAS LIFTING A SECTION OF CONVEYOR BELT IN ORDER TO RIG IT TO BE LIFTED WITH THE CRANE. HE BENT DOWN TO LIFT THE BOTTOM OF THE BELT UP TO SLIDE A CHAIN UNDER IT AND FELT A PULL IN HIS LOWER ABDOMEN/GROIN AREA.

The man was striking the drill head of the roof bolter with a hammer to dislodge a wrench when a piece of metal flew and struck the man in the eye. (The man is off work for additional treatment to his eye. No new incident occurred. Off effective 12-12-09.)

The miner operator was tramming the miner to a new cut and as the miner crossed over a dip in the mine floor, it slid and the boom contacted the operator's right leg. The miner operator's extent of injuries was a puncture wound caused by the miner boom. Sutures to close the wound was applied.

The driver just finished off loading his 1st load of blasting agant at the blast pattern & was getting the truck ready to travel to the batch site to re-load. He had removed the funnel from the end of the discharge auger & moved the auger into the saddle it rides in while driving & had picked up the funnel to place it in carrying mount when the auger came out and hit him.

Employee was carrying a 5-gallon bucket with tools in it and stepped off a 14"" beam with his right foot and his knee twisted and popped. Employee declined to see a doctor at the time. On June 8th, employee asked to be take to emergency room due to knee pain, has not been back to work since.

After setting the ATRS went to drill the first hole and rock fell behind ATRS and in front of ATRS. Rock fell striking just below left elbow cutting arm. Cause of this injury was safety awareness. Employee was using pressure washer to clean equipment when he decided to use on his boot. The wand slipped and went in his boot resulting in a burn to left foot

While walking to help with pump hose, employee's left foot slipped in mud causing his right foot and leg to fold under him breaking his right ankle and twisting his right knee.

As motor was exiting dip, operator saw man-trip on track ahead. Motor operator braked, but still bumped man-trip (no passengers). Operator of man-trip declined first aid, said he was not injured. Next day man-trip operator went to Dr. due to soreness, E.R. Dr. took him off work.

Miner was removing fitting off conduit (pipe), using two pairs of Channel Locks, using force to remove the coupling from the conduit and felt a pop in his left hand/wrist.

A 98"" x 24"" corrigated sheet slipped off a C-11R vise clamp falling approximately 30 feet striking injured in the right facial area. Injured received severe laceration from right ear to mouth.

A rock measuring 36""x31""x1.5"" fell from the mine roof, striking the employee on top of the head. This did not become a lost time accident until 5/11/09.

Employee was operating a continuous miner performing retreat mining, when a portion of the coal rib came loose and struck him on the back, knocking him down. He continued to work until 05/26/09 at which time he stopped due to the ongoing pain in his neck.

EE was pulling miners cable and throwing on the MBC and felt something pop in his lefty shoulder area. EE worked until the day of his surgery on 10/15/09. EMP WALKED INTO PATH OF MOVING FORKLIFT AND WAS KNOCKED DOWN RESULTING IN ARM BEING INJURED BY WHAT X-RAYS SHOWED AS A CRUSHING TYPE INJURY (SEVERE CONTUSTIONS/BRUISING).

Struck by falling object

Employee was positioning tire with axle of truck in preparation of mounting it when the tire began to fall back towards him. The employee was unable to support the falling tire when it began to fall. The tire knocked the employee to the ground with the tire resting on his left leg/knee.

Struck against stationary object

Employee was changing screen box and struck finger on bolt on 5/28/2009. Incident not reported to plant medical dept until 7/8/2009. While on vacation finger became infected required medical treatment.

Caught in, under or between running or meshing objects

Come-along holding belt up slipped, causing roller to move and catch finger between roller & belt structure.

Over-exertion (Not Elsewhere Classified)

The employee was using a wheelbarrow to move stone into the electrical blockhouse. Another employee was using a skid loader to dump the stone into the wheelbarrow, when the stone was dumped into the wheelbarrow, the employee was holding onto the handles of the wheelbarrow. When the stone hit the wheelbarrow, the employee felt pain in his right hip.
