Unintentional roof fall: d-intake, No 8 entry, intersection at No. 15, crosscut, 80 ft inby survey station No. 10013. The fall measured approx. 20ft long, 20 ft wide and 6 ft thick. Five foot fully grouted resin roof bolts were in use as primary roof support.
A roof fall occurred in the No 2 west entry on the No. 3 section speed 21253. The main roof fall approx. 9 ft above the roof bolt anchonge. The roof fall was approx. 20 ft side for distace of 30 feet.
The Cedar Creek elevator was taken out of service to replace slack rope indicator.
A roof fall occurred in the #9 seam mine in the East Mains in #5 entry in the pillarline between crosscut 26 and crosscut 27 measuring approximately 50ft. long x 18ft. wide x 9ft. high. This area was supported with 6ft. tension rebar bolts and square timbers.
A roof fall occurred in the 2 west 1, crosscut #75 between entries #4 and #5. The fall measured approx. 20 ft long, 18 ft wide and 10 ft thick. The area is supported with 5 foot, fully grouted resin bolts.
Roof fall discovered at 7+80, # 7 entry on the 6th East Panel. Roof abnormality, additional supports were installed.
Examiner found a roof fall in an inactive area of the mine. A fall approximately 20'L x 20'W x 10' H was found in the #3 entry at #9 crosscut of the 8 Haulage elevator mains. The fall is 11,838 feet from the nearest working section. The area was mined in January of 2003. No ventilation or travelway was disturbed.
An unplanned roof fall occurred, after evening shift left at approximately midnight and was found by midnight crew at approximately 3:20 am. Fall was approximately 50 feet long x 18 feet wide x 6 foot high and went from #6 entry towards #7 entry in the # 1 section at spad # 1055.
Conrols tripped and and needed to be reset.
A roof fall occurred in the #9 seam mine in the East Mains in #5 entry in the pillarline between crosscut 21 and crosscut 22 measuring approximately 50ft. long x 18ft. wide x 9ft. high. This area was supported with 6ft. tension rebar bolts and square timbers.
Mine examiner discovered a roof fall in the 8 South return at #5 crosscut in the #5 entry. The fall was in an intersection and measured approximately 20'wx20'lx5'h. The area is inactive and is 11,445 feet from the nearest working face. The entry was mined in August of 2002.
A roof fall meausring approx. 6'x20'x20' occured on the mmu 004-0 in the intersection near spad #4552. This area was outby section.
The Cedar Creek elevator was out of service from 6:30 pm until 7:30 pm due to a guide wheel was out of alignment.
The mine experienced a bounce/burst in the Gil 8 longwall section, MMU-010-0 at shield No. 12.
A piece of mine roof app 2 feet thick, 20 feet wide and 12 feet long fell on the MMU 002-0 retreat mining section in the first lift of initial #8 pillar block. No injuries to this occurence.
Had an unintentional roof fall on the 2nd pillar panel on west submains 40'x20'4'.
An unintentional roof fall occured in the 2C intake in entry #2 at station 6+75. The primary escapeway re-routed around the fall area. The fall did not affect ventilation or cause personal injury. The fall measured 40 foot x 19 foot x 8 foot. The area had been supported with 6 foot roof bolts prior to the roof fall.
During an MSHA inspection of the elevator at the Hart street portal, it was determined that accumulations of material on the elevator governor rope was preventing the brakes on the governor rope from properly stopping the governor rope. The governor rope was cleaned and the elevator was returned to service.
A fall of roof occured in the 2nd S.E. Mains in the #5 entry between spad #2+10 and 2+80. The fall was approximately 50ft long x 18ft wide x 8ft high.
Roof fall occurred on 001-0 MMU in #4 entry 3 crosscuts inby survey spad # 34505 at 1:50 PM. The fall was approximately 20' wide x 50' long and 6 feet in height. No ventilation was affected by the fall. This fall was investigated by MSHA and OMSL.
Drag system kicked out on overcurrent. Electrician was called to machine and reset. Within 5 minutes drag system tripped out again. Electrician checked back of machine and reset once again. Within a few minutes, it kicked out a third time. Electrician went to back of machine to check the drag motors and noticed fire coming from compartment underneath #1 Drag motor.
The electrical service in that area was shut off. 1 pallet had deep burns. It appears spontaneous combustion occurred in a bulk bag filled with 1/4 inch leonardite and soda ash. The moisture of the leonardite was approx 28%, higher than we recommend. We believe the combination of moisture, leonardite and soda ash caused an exothermic reaction. A chemist said this is possible.
No injuries, primary escape way blocked. Evacuated crews until escapeway was rerouted
Hydraulic line on west Sobo brake blows. Called MSHA took initial steps to evacuate the mine. Hydraulic line replaced and conveyance back into service.
Unintentional roof fall between 10 and 11 crosscut on #3-section. Fall size: 20'x15'x above roof bolt anchorage(5' ft. fully grouted rods) across from spad 607 and in the no. 3 entry.
A small fire occurred in the NTDF feed system located on the mezzanene level of the pre-heater tower, just below the 4th floor. There was minimal damage and no injuries involved with this incident. MSHA was notified and inspector conducted an investigation.
A fall of the roof occurred in the #5 unit in the 11th southwest mains in teh #3 entry inby, the tailpiece at X-cut #5. Length was 35', width 22' and thickness was 6'. Ventlation, personel nor equipment where involved or affected.
The a shaft man/supply hoist was out of service for a period of time for more than 30 minutes. A jog switch was stuck in the on position. The hoist was repaired and placed back into service at 5:30 pm.
A roofall measuring (50'L X 18'W X 7'H) occurred at spad number 6375in the SE NO. 3 Panel off of the NO. 3 NE Mains (001 Section). Bloted with 5' bolts. Area was mined approximately 3/2005.
Roof fall 300' outby the face in #7 off roadway entry broken wet shale roof.
Lost power to main hoist resulting in the reprograming of computer system. Power restored, computer reprogramed hoist resumed normal operation 2:20pm

An unplanned roof fall above the ancorage zone of the installed roof bolts. The fall is located at #18 belt (did not fall on belt) between 2 and 3 entry. The fall knocked a return stopping. No one injured or trapped, no damage to equipment.
An unintentional roof fall occured on the 4C travel road at 46 crosscut. The fall did not affect ventilation or cause personal injury. The secondary escapeway was re-routed around the roof fall area. The fall measured 30 foot x 30 foot x 12 foot high. The area had been bolted with 6 foot fully grouted roof bolts and 12 and 16 foot cable bolts.
A roof fall was discovered on 003 section while a preshift examination was being performed. The fall measured 40' by 18' and was 8' in thickness. The area was supported with 5' Hy-Tech bolts and 8' cable bolts. The fall covered up the continuous miner that was not in operation.
This is not reportable but the 800 number was called because information was not immediately available. The mine had a power bump that knocked the hoist and a fan down. When the fan was energized the fan ampguard shorted. The hoist was not damaged but not energized for about 1/2 hour waiting for an electrician to check.
When the fan ampguard had been rebuilt and energized it shorted and caused a ground monitor relay in the hoist circuit to fail. The relay was replaced, the fan ampguard replaced and the hoist returned to service around 3:00am.
A non-injury roof fall was found LOB of #4 heading into 4 right break. Fall measured 40' X 18' X 5'. Ventilation was not affected.
A roof fall was found at the #17 crosscut on the #3 belt track entry. The fall measures approximately 40ft long, 19ft wide & 8 to 10ft high. MSHA Inspector: AR: 23852
A roof fall occurred on the Slope Roadway at break #58. The fall measured 20' by 20' and was 4' in thickness. The fall was cleaned up and re-supported. No equipment or employees were involved.
Kuhntown elevator went down at 11:20 am until 1:24 pm due to a faulty door switch. No persons were on board at this time. Elevator back in service at 1:24 pm.
A roof fall measuring (40'L X 18"" W X 8'H) occurred at spad Number 6327 in the SE No. 3 Panel off of the MO. 3 NE mains (001 section). Bolted with 5' TT bolts. Panel was mined approximately 3/2005.
Brian Lester, Mine Foreman was traveling down roadway #5 entry and spotted a fall in #6 entry, 1 break inby spad #189. Brian made sure that there were no other hazards and that all men were accounted for...Both Federal and State Offices were contacted at appx 1:40 am--the fall was 20 x 20 by 5ft thick. The fall was cribbed at the intersection
Fall of ground (approx 95' X 45' X 0-5') occurred in 28E/8-9B of return escapeway, blocking travel. Strata was salt, crystal salt, & shale. Area bolted during initial mining w/ 8' point anchor bolts. No injuries or equipment involved. Persons were evacuated outby fall area.
Production hoist kicked to manual at 5:00 pm. Electricians investigated and reset production hoist controls. After several test runs, hoist kicked to manual again. hoist could not be controlled in manual. MSHA was notified and the mine evacuation began at 5:30. At 6:00 manual control of the hoist was reestablished and miners werer sent back to work.
An unplanned roof fall occurred in the #1 entry, Left return at spad #1031. The fall measured approx. 40 feet x 19 feet x 6 feet. The area was developed on 6/1/09. No injuries were reported.
A roof fall was discovered in the number 3 entry to 1 section. Location of the roof fall is inby spad 25957 adjacent to 5 c belt line, this area was mined 5/28/09 estimated size of the roof fall measured 6 ft high, 20 ft wide and 20 ft long. No injures where reported as a result of the roof fall.
A roof fall measuring (40'L X 18"" W X 8'H) occurred 2 breaks inby spad number 9435 in panel A off of the SE NO 4 Panel. Bolted with 6' bolts and 10' cable bolts. Panel was mined approximately 6/2007
A roof fall occurred in the 4 north 4, #6 to #7 crosscuts #8 entry. The fall measured approx 30' long, 17' wide and 5' thick. The area is supported with 5' fully grouted resin bolts (tension rebar).
an employee was doing a routine scheduled inspection of the Alimack, when the operating gearbox had a failure.
A roof fall occurred in the #11 seam at the inner seam slope panel in the face of the #3 slope measuring approximately 30ft. long x 18ft. 11 inches wide x 8ft. to 9ft. high. This area was supported with 6ft. tension rebar bolts, T3 Channels, wire mesh panels, 6ft. and and 10ft. post tension bolts.
Lost power and fan @ 7:35 a.m. due to heavy thunderstorms . All employees were out of the mine @ the time of the inundation. Reported @8:53 a.m. K-order was issued.
A roof fall occurred in the 1st SW Submain in the #8 entry intake escapeway at XC 9+30. This was in a slip area. The fall measured 30' X 30' X 8'. Some 9' cable bolts were in the intersection, but were pulled out or came out with the rock. Visible slips and wet soft bottoms that is present will require more support, such as cribs, timbers and prop-setters.
Door switch failed causing elevator to malfunction.
Roof fall approximately 73 feet long and 63' wide at it's peak was reported to the surface at 3:00 pm by an examiner. The fall was on 8O section which is in an inactive part of the mine. The fall was approximately 16,140 feet from the nearest working face.
Kuhntown elevator door would not unlatch to open. The elevator was on the surface with no persons on board shindler came and repaired the door latch and the elevator was released at 11:20 am door switch detective.
A roof fall 19' w x 760"" L x undetermined H was found in the return of 9 rooms from 2 xcross cuts outby station 6035 to 1 cross cut outby station 6229. Does not impede ventilation or travel
The fall occurred in AB Mains Section at the #2 heading, #14 crosscut and was 900 feet outby the face. No ventilation was distupted, and no equipment was damaged. The area was bolted with 6 foot Super Twist Bolts, 12 Post Tension Bolts, and screened. The fall was approximately 12 feet hight.
While taking the 1st lift of the #6 block on 1st rt panel, a fall occurred in the inby intersection at #5 xcut. The fall was approx. 20' wide by 20' long feathered from 2' up to 10' thick in the middle. Reason for late report: The MRS's were recovered & the mine was idled and put in inactive status.
Unintentional roof fall: No 1 Belt Entry, Intersection, Crosscut No. 11. The fall measured approx 20' long, 20' wide and 4' thick. Four foot conventional roof bolts were in use as primary roof support.
Roof fall in main north. 35 block in the #1 entry 5' to 6' high, 14' wide and 20' long. This is an old area to the mine and a walk way between scals approachs have been succrred and dangered off.
An unintentional roof fall occured at spad # 410 on Red No. 1 Mains, # 2 entry approximatley 900' from working section. The mine is idle at this time. The fall measured approximately 30' long x 20' wide x 8' high. Fall will not be cleaned up.
A roof fall was found by our examiners on the 3 West C roadway at crosscut 136. The fall was located in the intersection and blocked travel thru the area. The fall will not be cleaned up and the roadway will be re-routed around the area with additional supports installed in the area.
Hoist was down due to the emergency stop button.
6th 48 Beltline, X-cut 44. Width 50', Length 16', Height 6'
An unintentional roof fall occurred at the above time and date. Fall located on 11 belt, 19 block, 5 hdg, station 814. Fall measured approximately 18'W X 50'L X 10'/15'H. Continuation of fall to the the left of current fall. Previously supported with 6' full grout resin bolts and cribbing. Area mined on or about January 2007. Overburden at or about 180'.
A tailgate blockage occurred at approximately 12:45 am. No injuries or property damage occurred. Ventilation on the longwall face remained within plan parameters.
Roof fall was discovered in the West Mains #5 entry at #48 cross-cut. Fall measured 45 feet long x 14 ft wide x 6 ft high. Area had been bolted to plan. Area was re-supported with wooden cribs and pack setter posts. No injury's and no equipment damage and did not block escape ways. Roof fall was immediately called in the MSHA and WV hotline.
A roof fall occurred in the supply road of the 1st main west a xc17 the ventilation was affected. New stoppings were built and ventilation was re-estability. The fall was 30' long x 8' high x 20' wide. The fall was discoved at 2:30 pm.
On 6-12-09 the mine examiner found a roof fall on the old #1 Section.
Fall in the # 5 belt entry at spad no. 4101 fell in the intersection on the # 2 section. It was approx. 8' to 10' high. It was cleaned up and rebolted in the area. No persons or equipment was affected.
Lost SWEPCO power on both hoists due to component failure on SWEPCO side. Miners were not evacuated due to the unavailability of an electrician to operate the emergency generator. SWEPCO power was restored at 6:08 AM and the hoist was tested and returned to service. Miners returned to the surface at the 7:00 AM shift change.
An unplanned roof fall occurred in the #2 gas mine at #6 brk, from #5 thru #7 entry at spad #541 that measured 140' long x 18' wide x 10-15' high. There were no injuries reported. The area was developed on 3/30/09.
Anomalous gas readings indicated the presence of methane and carbon monoxide behind new seals installed in the west mains of the mine. It was found by MSHA that the glue used to couple the seal gas testing tubes emitted fumes which the detectors read as methane and carbon monoxide. Air samples were pumped from behind the seals and results indicated no methane or carbon monoxide
An unplanned roof fall onto the no.1 conveyor belt approx. 200 ft inby belt portail. The face was approx. 4 feet in length, 20 feet in width, and 3-5 feet in height.
An unplanned roof fall occurred in the #2 Gas Main in the # 16 entry, # 10 brk, at spad # 575 that measured 90' long x 19' wide x 10-15' high. No injuries were reported. The area was developed on 4-10-08.
An unintentional roof fall occurred in the No. 1 return entry at the drift opening. The fall measured 5' long, 16' wide and 9' high. The fall does not affect the air passage or the travel of men.
An unplanned roof fall occurred in the #2 Gas Mine in the #3 entry, brk #17, at spad # 693 that measured 90' long x 19' wide x 15' high. No injuries were reported. The area was developed on 4-23-08.
At approx. 3:40 am, a roof fall occurred in an intersection on No. 2B supply road in the No. 6 entry. The fall was approx 5,800 feet outby the working face. In addition, the fall was approx 30 feet long, 15 feet wide & 9 feet thick. It did not affect ventilation or passage of men. It did not fall above the anchorage of the bolt being used. Additional Cribs & Rails will be installed
A roof fall occurred in the #11 seam mine in the 2 East at crosscut #31 in the pillarline between #7 and #8 entries measuring approximately 15ft. x 15ft. x 12 ft. high. This area was supported with 5ft. fully grouted roof bolts and truss bolts.
A roof fall occurred in the 3N3, #45 crosscut, between #8 and #9 entries. The fall measured approx 25' long, 18' wide and 6' thick. The area is supported with 5' fully grouted tension rebar bolts.
Roof fall in #27 crosscut that rolled out into track entry. The track is inactive and is only traveled by examiners. The fall measured approximately 10'L x 18'W x 6'H. The area is 33,820' from from closest active face and was mined in June 1972.
Rock fall occurred in 3 East Mains adjacent to 9 belt at break 1 between #10 entry and #11 entry. Fall measured approximately 18'-20' wide x 50' long x6'-8' thick.
EE, Mine Foreman was making the required weekly examinatin for Non Producing Status when he discoverd a fall had occured in the Intake--3crosscuts from spad #257--appx 25ft long and 19 ft wide and 2 ft thick. This fall was reported at appx 8:52 am
An unintentional roof has occurred on the no.5 non-producing section outby in the 4 entry near spad 500 at break 12. The fall measured 20x20x8. Fall above anchorage of bolts. 5' foot mechanical anchor resin-assisted bolts. Will not be cleaned.
Two EE's were cleaning in Rt Propel. Sending 5 gal bucket to top. Rope went slack. Found two employees laying on floor in grease. Taken by ambulance to Medical Facility in Austin...tested and found nothing damaged.
The elevator at the #2 portal became inoperative. The govenor switch needed adjusted. No one entrapped. Elevator running at 1:05 PM.
Rock fall along the 1st northeast main in #1 entry. Location is from spad #23+36 to #24+06. Fall size is approx 100' X 19' X 8' high. This area is 9,000' from the nearest unit.
A large storm producing rain at 6 to 8 inches per hour took place on the evening of 6-8-09. This caused accumulations of water in the pit structure of the slope construction project. Rain and run-off water eventually flooded the face of the slope. This report is being submitted in conjunction with a violation issued 6-25-09 which officially determined this event an accident.
A roof fall was discovered by the belt examiner, at break 5 on 2 belt, the esimated size of the fall is 20 feet wide, 20 ft long and 10 feet high, this area of the mine was developed between 1978 and 1979.
The elevator at the Camp Creek portal was down for more than 30 minutes due to a bad mid-shaft switch. The switch was replaced. Persons could travel the slope by slope car or walk.
A roof fall was discovered in XC-39, between entries 1 & 2 in the LW-304 tailgate. This fall is an extension of one that occurred on 6/05/09. The fall was rib to rib, measuring approximately 19ft W by 9 ft H. The length could not be determined as it closed off access to this x-cut. The next isolation stopping was installed to remove this from the active workings.
The Tyro Creek Elevator was out of service from 5:45 am until 1:45 due to a car overload fault.
Hoist was down due to an electrical power supply was down.
The roof fall occurred at 20 Crosscut, 2200 feet outby the face. It was approximately 20' x 18' x 6'. No persons, equipment, or ventialtion was affected.
The continuous miner was cutting the second push on the block between 6 and 5 between 8 and 9 entry, when the roof fell on the continuous miner. No equipment damage and no injuries. Recovery was completed by 6/6/09 5:30 PM.
A hay bale setting next to a county road caught on fire. It is on land leased to a local rancher. It smoldered for more than thirty minutes.
Hay bales stored inside a fence just off a county road in Lee County Texas were on fire. The cause of the fire is unknown. Local fire departments from Lexington and Blue Texas where called, and extinguished the fire. There is no injury associated with the fire and the property damage is limited to the loss of 10 round bales of hay.
Roof Fall: Outby spad 144 30' long, 19' wide & 4-5' thick. Ventilation not blocked. Primary escapeway blocked.
Fall occured 60 ft inby spad # 10489 in the bleeder system on the North West U submains. This fall is in the process of being sealed. MSHA inspector investigated this fall.
#3 hoist tripped at 7:45 pm with ""Drive converter a"" fault. Electrician came out of the mine and repaired a contactor and reset the hoist. Hoist was back in service at 9:00 pm. There were still two escapeways during the incident.
During an inspection an area in the tailgate was discovered where the roof had fallen between the Cans and the rib. The entry remained open. The fall was approximately 7 feet long by 7 feet wide and went above the anchorage of the roof bolts.
An employee entered a surface elevator to get to the top floor of the SC1 Drive House. The elevator stopped functioning and the employee riding the elevator was entrapped inside. he was there for approximately 90 minutes from 9:00AM to 10:30AM.
Discovered a roof fall in the #5 room on the HL1 panel 1 break inby room spad #3115. Fall is 5' thick shale x 19' wide x 19' long and was bolted with #5 x 48"" fully-grouted resin roof bolts. This area has been dangered off.
When traveling air course, a rock fall was discovered in the 1 Rt. Panel off of the 1 Left Submains Bleeder. The fall is approx 18' wide, 5' high and 30' long. The area has been cribbed and timbered off. The fall is at Spad 2461.
The #8 elevator failed to leave the top landing. The elevator displayed contactor fault. Upon investigation a faulty coil on the M contactor was found. The coil on the contactor was replaced and the elevator placed back into service @ 9:15 AM. No individuals were on the elevator at the time.
A fully loaded CAT 776C water truck with a 20,000 gal. tank was traveling up a ramp made of coal, when the truck lost momentum and started to travel back down the ramp. The tech engaged the retarder and the truck came to rest on its tail end.
A roof fall measuring 30' long x 20' wide x 7' thick was discovered @ 2:35pm on 6/5/09. This area was mined in January of 2009.
A roof fall in old 4 west right return at 18 block, x-cut 8 to 9, above the anchorage bolts. Fall was between spad numbers 8355 & 8256. This fall will not affect ventilation or walkway. Approaches were posted and dangered off.
Roof fall, 60' over to right of spad 130, #8 entry intake. 50' long X 19.5' wide X 5-6' thick. No passage way blocked, no vent change, no escape routes blocked.
Roof fall occurred in intake escapeway #4 entry blocking passage to the outside escape route, roof fell 10 to 12 feet above roof horizon at a four way intersection. Fall was 16 feet wide to 35 feet long.

A roof fall was discovered during an exam. The fall measured 20' by 20' and was 5-7 feet in thickness. The fall was located in the 1-Left bleeder entry off B-3 Mains, #4 crosscut, between the #2 and #3 entries. The fall was dangered off.
This mine experienced a non-injury roof fall in the number three entry on 001 MMU at spad # 4559. This roof fall trapped the section feeder. The fall measured 20 feet X 20 feet X 12 feet.
Kuhntown elevator went down at 9:22 AM with 20 EE's on board. EE's were on the elevator for 68 minutes. Shindler came to repair the elevator. The problem was that a screw came out of #2 blow out coil. Elevator was checked by Schindler and MSHA then released for full use at 1:38 PM.
Unintentional fall of the mine roof located in the #3 entry at crosscut 98 on the 9 left panel at spad # 202. The fall is 19 ft wide, 10 ft high and 90 ft long and is 550 ft from the working section.
The top portion of the A side of the plant washer caught on fire approximately 2 hours after hotwork (welding/cutting) had completed in that area. The fire is believed to have been caused by a piece of slag or burned metal that came in contact with unknown combustible material which may have smoldered and eventually caught fire. A detailed investigation is ongoing.
An unintentional roof fall occurred in the No. 7 entry at spad No. 2625. The fall measured 30' long, 20' wide and 3' high. Air passage or the travel of men was not affected.
On the hoist, the connecting rod for the break snapped at the head and the hoist was down until repaired, the crew was pulled from the mine.
An unplanned roof fall occurred at Knight Hawk Coal LLC, Royal Falcon mine on 6-2-2009 in the Main North return. MSHA was immediatly notified and all steps required were followed. No K-order or citations were issued since it was in the return outby the tailpiece. No injuries or equipment damage happened in this fall.
Roof fell approx 1700 ft from active face of #1 section in #3 entry. Fall was approx 30 ft in length, 20 ft in width and 7 ft in height. MSHA Inspector Billy Stiltner investigated the roof fall.
An unintentional roof fall occurred on the 4C return at 9+80 station in the #2 entry. The roof fall measured 24 foot x 25 foot x 6 foot. The fall did not affect ventilation, passage of men or cause personal injury. The fall occurred in a 4-way intersection. The fall had been bolted with 6 foot roof bolts.
@2-5th bletline (X-Cut 5). Length 18', width 18' and thickness 9'.
During shift change (from 1st to 2nd) the Koepe man cage malfunctioned. Investigation revealed brake switch adjustment and cannister change needed. 8 persons in mine evacuated via production shaft. No injuries; or other problems.
A fall was discovered at approximately 8:32 AM in the #4 entry on the Laurel Mains. The fall is approximately 18' wide X 20' long X 6' high. Located approximately 2700' outby the active working area.
Rock fall occurred in 1 East Mains in the #3 entry at break 48. Fall measured approximately 20' wide x 40' long x 4' thick.
Severe weather started to move into the area around the coal mine and within a short period of time heavy rain began. The area around the highwall and surrounding drainages began to run a considerable amount of water. The water inudated the reclaim tunnel area under the coal stock piles. Miners had been evacuated from the mine.
A rock fall was discovered on this date on 005 section. The fall started near the gob line during retreat mining and extended outby into the intersection at CC #51 in the #4 entry and extended into the crosscut toward the #3 entry. The fall caught the tail of the continuous miner that was parked in the crosscut. The fall ocurred during an idle shift with no one present.
The west brake switch on the Koepe access cage malfunctioned. Troubleshooting finally determined the problem and the switch was replaced. Mine evacuated; 27 persons in mine. No injuries or further problems. Third shift did not enter mine. Hoist back in service at approximately 12:50 - 1:00 am.
Unintentional fall of roof located in the # 7 entry at 47 cx on the northwest section panel at spad # 954. The fall was approximately 19 ft wide, 10 ft high and 70 ft long and is 1000 ft from the working section.
An unintentional roof fall occurred in the Old #3 worked out panel (1st West off of the 1st North Sub Main) at 20+00 station entry #6. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 20foot x 20foot x 6foot high. It occurred in an 4-way intersection and will not be cleaned up.
Between 5 am & 5:30 am on Monday 6-1-09 it was discovered that there had been a fire in the air compressor room over the week end. It was ok at end of shift 10:30 pm friday 5-29-09. Fire had burned itself out. The area was secured & proper authorities contacted. Fire Marshal ruled a heater caused it & we are waiting his report.
The elevator serving #6 Portal was unavailable due to a malfunction in the door lock. The door lock was repaired and the elevator was put back in service at 1:30am on 6/2/2009.
Employee was in the process of setting or had set a roof jack, on the left inby corner or #60 Crosscut between the No 2 and No. 1 entry in the 4 west miner section. A piece of stone fell out of the roof and struck the employee, pinning him to the ground.
The investigation is not complete, but as of now it appears that the employee had washed the #2 drill and was rolling up the hose when a portion of the mine roof collapsed, striking and covering him with a section of the salt roof.
Employee was looking for hydraulic leak and was struck in head by eject all plate. Employee received a massive head trauma and later became a fatality.
Supply car was bringing concrete block into break to build permanent stoppings when chain broke letting supply car roll through curtain crushing foreman between supply car and 94L bridge causing fatal injury.
Surface Mine Engineering Sampler was backing up his flatbed pickup on a a drill pattern. A Drill Operator had dismounted his drill and was walking on the pattern. The drill operator walked behind the pickup and the sampler was not aware the the driller was there. The driller stopped walking and was run over by the flatbed pickup, resulting in fatal injuries.
While feeling to verify that eyelets were lined up the injured employee had his fingertip pinched off when another employee lowered the inner link. The right index finger was pinched off between the back of the nail and the first joint.
Employee was taking a chunk of shale out of bin when left hand pinky got caught in gate. 1/2"" of top part of pinky finger came off.
Employee was kneeling down picking up handtools when a shuttle car ran on top of him.
Injured was cleaning up accumulations of coal with a scoop tractor and backed up to turn into a crosscut. The persons hand was positioned outside of cab and while pivoting, the machine caught his hand between the cab and frame of machine. The man amputated his middle, ring and small finger of his hand to the second knuckles.
Employee was working on continuous miner on mmu n0/002 section. He and helper had to remove large lid on miner to gain access to trouble shoot and repair. Large lid fell while re-installing pinning the employee s hand between the lid and machine. This resulting in the 1st joint of the 3nd finger on the employees right hand being cut off.
Alleged groin pain/discomfort from changing out screens on screen deck, possible hernia.
While using a floor mounted grinder to smooth up a cut on a thin piece of metal the object and the employees right hand were pulled into the wire brush wheel. The table and guards were all in place and properly adjusted. The employee sustained an amputation on the tip of the little finger and a laceration on the ring finger which required 12 stitches to close.
EE was sitting on a stopped scoop when the operator inadvertantly moved the bucket down his left hand was caught between the frame of the scoop and the ram causing a crushing injury to the left hand and amputation of left middle finger.
Employee was exiting a truck when he slipped on the running board causing a meniscus tear to the right. ***This is a reoccurance***
While adding drill steels, employee wanted to swing bolter head to clear away loose rock and coal to gain more ground clearnace. Instead of hitting swing lever, he accidently hit up lever.
On 7/1/09, employee called supervisor to report that he was not coming in due to lower back pains. Employee thinks that while operating the dump loader on 6/30/09, the jerking action while driving and hauling materials from Phase II to the Primary Crushing unit, is what caused his back to be sore.
While hanging curtain in #3 entry face slipped and stepped in a hole in mine floor sprained right ankle. Cause is concentration being not aware of mine floor conditions, Compliance of rules/regs not a factor, mining equipment/system not a factor, ppe was not a factor, miner profiency was a factor.
Stacked sack fell over while moving, pallet hit Injury's left leg.
Employee was attempting to remove a broken bolt from a bucket pin on a front end loader. The chisel slipped as he was striking it with a hammer and the hammer struck his left thumb. The employee recieved a nondisplaced small distal tuft fracture of the left thumb.
Employee was pulling shields and became stuck in the mud and twisted his left knee. Lost time injury 08/18/09 due to surgery.
Twisted lower back. Employee was holding one end of a spreader, the other end was unhooked from the crane and dropped.
EE tried to shift to a different gear, couldn't get it back in gear. Then couldn't stop the truck.
Employee was in the feeder tunnel removing material from the conveyor chain. A rock fell from an undetermined location striking him on the right side of his head.
Employee was helping remove a large rock that was stuck on the feeder belt. When trying to get the rock in position on the belt it rolled and employee had his hand on belt and rock rolled on his hand causing a contusion to right hand.
Employee was wearing an arc flash protection suit, he was racking out a 12470V breaker to be locked out. After the employee racked out the breaker he removed his glove to open the door to put a lock in place. At that time employee claims that he was zapped by something.
EE was nailing divider deck when he fell through the decking landing on his right elbow causing a contusion.
The employee was mounting his truck when he fell backwards after losing his balance. The employee struck his left heel on the ground causing a strain in his heel.
EE was ascending side slope of a limestone stockpile to get elevation of top of slope. He lost footing of lt foot, started to slide. Trying to catch himself his rt leg twisted & collapsed under his own weight. The rt femur was broken at a location of a previous break. According to EE, the bone was previously broken 4 times due to a degenerative bone disease.
EE was going up a silo in a man basket. As he reached the work level he prepared to put on his blast hood. He noticed his rope grab was sitting on the basket & his life line had not gone through. As he started to feed the life line through it whipped back & hit him in the right eye. The strike ruptured blood vessels and tore his retina. He was not wearing his safety glasses.
EE was cleaning area of tail wheel when he went to move, foot got stuck and twisted knee.
EE was doing maintenance on a screw when EE tripped on a small propane canister and fell approximately 5 feet into a hole in the decking, causing bruising and trauma to the left rib cage.
Employee got too close to berm on right side striking berm, losing control & crossing road striking left berm before regaining control of rock truck. Hit right arm on glass causing laceration. He bruised hip on seat frame. Employee told company official & foreman he was driving too fast. Employee tested positive on drug test.
Employee stated he placed his left hand on the continous miner on the 6 right section and was struck by a piece of rock that had fallen from the top approx. 12"" x 12"" x 1.5"", resulting in a fracture of the left little finger.
Employee was bolting top when a piece of draw rock fell and struck him on the back of his head.
Employee claims that while he was taking down water line a piece of the line fell and struck him on the top of the head. ***Lost work days began July 1 2009
The miner was removing rollers and frame. The frame slipped and caught two fingers of left hand between frame and bed rail, cutting 2nd and 3rd finger of left hand.
The miner was working as a catcher on the hydrolic stone splitter. He had caught a rock and as he was moving his hands away the next rock was split leaving the blade in a straight out manner instead of falling down. The rock struck the miners finger requiring 12 stitches.
Employee was lifting hydraulic pump and strained lower back.
Employee mounted a ladder and did not secure it. The ladder shifted and the employee fell to the ground. Employee was transported to CAMC General Hospital and x-rays were negative and referred to a specialist. Employee saw another doctor who advised that he had a compression fracture at L-2 and this accident became reportable 7-20-2009.
HIT IN RIBS WITH PIPE WHILE PUSHING COOLER DUMP ARM ON
Injury was measuring the distance between 2 bolts while rib screening, when he turned to walk back to pan line the rib rolled striking him in back pushing him into pan line. Cause of injury rib roll, mining equipment was not a factor, compliance with rules/regs not a factor, ppe not a factor, miner profiency maybe a factor.
EE states he was in a squatting position grinding metal, when he felt back pain and hip pain when standing. EE sought medical treatment on 7/2/09 and was taken off work by his treating physician.
Spot bolting between spad #1425-1426. In 3 Lt one break inby feeder rib started to roll out causing co-worker to move to avoid rib hitting inch trap causing bolter to move and run over employee's left foot.
Location B9HG #3 Entry, 6XC. Personnel were shoveling ribs, a piece of rock (approx. 3x16x18"" in 3 pieces) came loose and fell from the roof and rib line. The rock struck the employee on the left foot. Injuries consist of soft tissue tears to the foot, scraps and contusions to the left knee, left neck, shoulder and left hand.
Washing down sump with a water hose while standing on a platform. Slipped and fell approx 18"" striking back on a crib block and handrail.
Employee was walking on the North MFL pad, when he stepped into a hole. When he stepped out of the hole, he heard a pop come from the back of his left ankle and felt numbness.
Employee stated that he was helping mechanic to change the teeth on a loader bucket. Mechanic cut teeth off with cutting torch and employee was knocking teeth off with a hammer. A piece of metal slag flew off, struck and embedded in his upper left arm.
Miner was walking to his work area tripped and fell.
Employee was loading a truck when dust and fine pieces of rock came back on him hitting him in his right eye. His right eye lid was bruised but no apparent damage to his right eye.
Moving cutting edge in storage area. Paint caused two (2) new edges to stick together & lifted both - straining shoulder.
The injured party had climbed on a loader to use it. The loader had just been pressure washed. Injured party was cleaning the windshield, when he slipped and fell.
While greasing, employee removed part of a guard. Employee's glove got caught in tail pulley and pulled his hand into a pinch point. Employee received broken bones and lacerations to the left hand.
EE missed a step when he was deboarding a ladder and fell to the ground.
Employee was replacing a broken U-bolt on the rear suspension of a Triaxel. As he was hammering on the broken U-bolt after using a torch, a ""bb"" size piece of slag struck his left wrist and became embedded in his arm.
Employee was in the process of splicing a cable when the twisted cable slipped in his hand as he was splitting the jacket of the cable. The knife blade struck the palm of his hand cutting it open in a slicing motion.
While he was tramming from the crosscut into the #2 heading, a rib rolled into the operator's deck. The trailing cable had cut into the coal rib over a period of time, causing it to become dislodged.
While bolting, a piece of draw rock fell and cut right arm, above elbow. Resulted in 14 stitches.
Employee sustained low back strain injury when the excavator he was operating lifted slightly off ground and then came down hard, jarring employee.
While tramming a Fletcher bouble boom roof bolter from the deck, the employee grabbed the canopy and mashed his right hand into the roof. He received a laceration on his index and middle finger. The employee did receive sutures.
Employee was walking by a ballast car when he caught his right jacket sleeve on the door release wheel, pulling his right arm backward. The event became reportable when he began missing work on 7-27-2009.
Was helping to replace a head pulley guard, pulled groin, possible hernia. Accident happend on 6/25/09, reported on 7/2/09
Loader operator climbing down ladder, foot slipped off bottom rung. Employee fell backwards and hit his head.
EE bent over to pick up some trash, when a small piece of coal sloughed off from the rib and stuck him in the right knee.
Employee stated that as he was exiting the truck his foot slipped off the step and he landed on the ground in an awkward maner. He later stated that his knee was sore. He was taken to the clinic as a pre caution where it was diagnosed as a minor strain. Return to work regular job.
Employee was Placing 100# sacks of Bentonite clay on pallets. As he grabbed a full bag and was turning to place the bag on a pallet, he felt severe pain in his lower back.
Employee used foot to move a bag on a conveyor belt without stopping the conveyor. Shoe lace was caught in the conveyor. Result of the accident was a scrape and bruising to the top of employee's right foot.
Employee stated he was operating rock truck. He stated the rock truck bounced & his neck & left arm started tingling.
EE had put a fan hub back into the block housing and was tightening bolt, when he felt a pull in his back. The next day he was diagnosed with a lumbar strain. The following week was vacation week. He was to RTW on July 6 but his wife called that day to say that he was to have surgery. I spoke with him today and he is taking therapy right now.
While getting off the top of a scoop, he stepped on a 4x4 timber causing the fracture.
Employee was unloading a rail car in the 3B section. When lowering the ramp on the car it slipped out of his hands and pinched his left index finger between to ramp and the stop block. Employee was wearing gloves. The injury resulted in cummutative fracture to his finger.
Surgery for Left Inguinal Hernia, employee taken off work.
Employee was moving a 1.75"" x 40"" circular steel part. The part was waist high resting on a pallet and was supported by a forklift. Employee slid the part to the side and it tipped over the edge. While the employee was easing the front edge to the floor the opposite side fell off the forks and struck the employee's left foot causing a hairline fracture of the metatarsal.
Scoop operator was taking the scoop to the charger, it quit tramming in forward-put in reverse-stuck in tram, went to hit Panic Bar, scoop hit rib. Threw him partially out-got hand behind the rubber stopper. Mashed his little finger. His little Left hand finger required stitches.
EE was changing out a tram motor on a shuttle car. As he was rolling the new tram motor into place, he felt a pain in his lower back. EE was treated for a back strain and released to return to work untill 10/18/2009, at which time he had a MRI and back surgery for a herniated disk
Employee was walking across unit and stepped on a rock twisting his knee.
Injured EE & another EE were preparing to off load vulcanizing tools from a Johnson Industies 4 wheeler onto a rail mounted flat car. As the other EE was backing up to the flat car, injured EE stepped between the 4 wheeler and flat car, pinning his left leg between the two. Resulted in a fracture to his lower leg.
EE was raising drill head, when fast feed lever stuck causing drill steel to bend, striking his left hand.
Employee was lifting a vulcanizing platter off the belt onto a trailer and strained his left side.
While operating a haul truck, EE was attempting to locate a ""rattling"" noise which was coming from the rear discharge vent of the AC unit located inside of the passenger jump seat. EE inadvertently inserted his fingers into the vent while ""feeling"" around for what was making the noise. EE had 3 fingers contact the blower, which cut the fingers and lost nail of his right hand.
Lots of rain, muddy and wet.
Foreman/Operator had been checking JD 400 for mechanical problems, he was coming down the ladder and missed the last step and fell.
Employee was riding a mantrip that ran over a submerged crib block. Crib block flipped up striking employee in lower back.
Drill operator stated that he stepped in a crack on the bench at night and injured his knee.
Employee indicated she was on her knees when she threw a crib block and felt pain in her lower back.
The employee was lifting a bucket of plaster up to put on top of the jeep, when he felt something pop in his shoulder.
Employee was loosening a bolt off a loader when the wrench broke, came back and hit him in the right eye.
The employee was lowering a track air lock door into a scoop bucket by hand when he pinched his right ring finger between the door and the scoop bucket. The employee began losing time as of 6/30/09.
On June 23rd of 2009, an employee felt pain in his right knee while moving a ladder. An MRI showed an accumulation of degenerative changes including tears in the meniscus, and other chronic medical conditions that required corrective surgery on August 31st. Because of the chronic, cumulative, and degenerative nature of the changes to the knee, we consider this to be an illness.
Injured EE was using the manlift to service a hoist. He let the boom down on a piece of pipe, lifting a wheel off the ground. He fell from the manlift bucket but was caught by his fall protection.
Employee was assisting co-worker in putting a bolt in the roof. He was keeping the drill in line while pushing the bolt up when the bolt slipped off the drill head and caught his right thumb in between the bolt and the drill head.
Injured began losing time when he was diagnosed with a torn left rotator cuff tear and surgery was necessary to repair it. He is an underground mechanic and the repetitive motion necessary to perform tasks for this job contributed to it. He began missing time on 7/27/09.
Employee was washing out a repulper screw that was stopped up. As he started washing, he inhaled Aqua Ammonia vapors from the product.
EE was installing the second bolt from rib on last row in #3 entry. Had bolt in hole, trying to align bolt head with drill chuck. Used rotation to align, plate spun and cut EE's forearm.
Employee walking from vehicle to the office to begin shift & tripped on the drain. He fell injuring his left hand, fracturing his outer-most metacarpal bone near the joint of his little finger. Please note that the reportability of this accident was not determined until 7/31/09 when the operator received additional information proving that an injury occurred on mine property.
On 6/23/09 EE alleges an injury to L/shoulder walking on a ramp, lost his footing and fell to the ground. The claim was questionable due to several issues. A lengthy, comprehensive investigation of the validity of the claim was completed 10-09-09. Further info will be communicated to the local district office.
Injured mechanic pulled on the latches on the battery box cover on mechanic truck and felt pain in right forefinger.
EE was lifting a piece of wood off of the hopper and when he turned the wood caught onto the hopper and he lost his balance and fell.
EE stated that as he was dismounting his equipment at quit time, his shoulder felt as if it popped out of socket. EE was taken to Emergency room and results were negative. EE went to his personal DR. on 06/24/09 and we are waiting results from test. EE had surgery on this shoulder in 2008.
Loading dust in scoop bucket. Foot caught between stinger and bucket. Mashed foot and sprained knee.
The MBC dolly had been loaded into the scoop bucket to facilitate the haulage system relocation for the section move. As the tramming operation began, the dolly shifted in the scoop bucket pinching the victim's index finger of his right hand which he had placed on the bucket of the scoop resulting in a laceration/crush/fracture of the right finger.
The employee was in the process of installing 500 lb castings inside a kiln when the casting shifted, striking the employee in the right leg and breaking it. The work area was clean and free of debris and well it.
Employee was filling in for absent RB operator & had bent bolt to install, and was straightening it. Bolt hd. & chuck not aligned. Employee bumped rotation to align bolt & chuck; caused bolt to spin - plate struck right forearm caused injury. Employee taken to E.R. for treatment.
While operating a Joy 21SC shuttle car, EE ran car over a rock measuring 13""x6""x7.5"" causing him to jam his head into the canopy of the shuttle car. He reported he was having head and neck pain and was transported outside and then to hospital by ambulance.
The injured twisted his right knee after stepping on a piece of roof rock on 6/22/09, he has continued to work. An MRI conducted on 8/20/09 shows the lateral meniscus ""degenerated severely"", and the medial meniscus is also abnormal due to degeneration.
Miner was lifting and carrying a gas powered 2"" pump when he stepped the wrong way and felt pain in his right calf. This resulted in an acute right calf strain.
While greasing the tail section of a conveyor belt at the #1 mill the employee slipped and fell from the level concrete slab that served as an ankor point for the conveyor.
Employee was in the process of cleaning the section dumping point with a battery scoop when the scoop was inadvertently struck by a shuttle car. The employee struck his head on the canopy of the scoop.
The E. was attempting to replace a trough roller structure on a conveyor. The E. placed the roller structure onto the conveyor frame and was attempting to put it into the upright position, between the belting and frame, when his thumb was ""wedged"" between the two. The E. experienced trama to his thumb and drove himself to the E.R..
Pulling air lock doors open and strained lower back.
As EE was walking down the belt line, he struck his head on a fire outlet that was hung from the water line, thus jamming his neck. EE continued working until 7/3/2009, at which time he went to the doctor. He was put on lost time on 7/20/2009.
Raising a conveyor with a cable ring gear style crank. The handle of the crank slipped out of his hands and the handle hit the top of his left foot, causing a metatarsal fracture.
Employee was drilling the top level tap on #3-3 flash tank when the drill bit got stuck. The drill then back spun (counterclockwise) and it was at this time that employee felt a pain to his right shoulder. EE taken off work on 08/20/09.
Employee was walking along the supply trip when he slipped catching his left foot under the rail. He fell forward and received a contusion on the top of his left foot.
while dismounting a dozer the employee slipped and fell, straddling the dozer arm to the blade
worker was walking down an incline when he tripped on a rock. He put out his right hand to break his fall striking it on another rock. He suffered a laceration on the palm of his right hand that required four sutures. Fatigue and an uneven surface contributed to the accident.
Felt pain in low back after hanging bolt machine cable. First day missed was 6/24/09.
During a pre-shift inspection, the operator was stepping onto a dragline bucket when he heard a pop and sprained his left knee.
While unloading shuttle car tires off of a dolly, his middle finger on his right hand was pinched between the dolly and the tire.
Employee states that he was moving rocks in preparation to bolt a cut when he felt a pain in his right groin. He worked his regular job until the scheduled miner's vacation (June 29 to July 5). Employee was disabled by Dr. Bucci effective 7/6/09 for a right inguinal hernia.
Removing a bearing from the housing by striking a hammer with another hammer. A sliver of metal from one hammer struck him in the abdomen.
Employee was greasing equipment when the hose of the grease gun came loose from the fitting. The end of the grease hose struck employee in left eye.
Employee was injured while placing a 2x4 on the ground to elevate a rock. The rock came off of the table. When it landed the 2x4 shot out from underneath and hit the employee's right ankle. Today 6/23/09, this was reported to me because the employee wanted to seak medical attention. He finished work on friday and also worked monday 6/22/09 all day
Employee was pulling belt over to splice into set up belt when he felt pain in his right shoulder.
Employee was in the process of washing out the scrubber duct on a miner and had lifted up a metal cover to spray inside of the scrubber duct. As he started to wash out the scrubber the cover fell landing on his arm causing a fracture.
Member stated that he was hanging the continuous miner cable across the roadway, when he felt pain in his arm (R). He did not report this incident to anyone, but went to doctor the next day and was taken off work by doctor until 7/8/09. Mine management was informed on 7/6/09 about the alledged accident, which no report was filed.
Victim was driving the LT8000 fuel/lube truck unit #LT01 west on the BB access rd to service CAT D9R dozer unit #DZ04. Victim blacked out a few seconds before reaching the dozer. LT01 impacted the right side of the dozer throwing victim against shoulder strap, seatbelt and steering wheel of LT01. Victim never applied service brakes.
Employee was changing a hydro senser in the hydraulic system of a end dump. Employee had bed raised and secured. He had released the pressure from the tank. When he removed the sensor, hot oil sprayed on his hands resulting in a 2nd degree burn to his thumbs and 1st degree burns to his hands. Unit #1155.
EE stated that while lifting a miner cable, he injured his back.
Employee was standing in mud hanging a high voltage cable. When he attempted to move, he twisted his left knee. Lost time injury due to surgery on 8/24/09.
Employee was walking under the diester screen, when a rock fell off the belt and landed on his left foot, resulting in a fracture.
Employee was holding plastic in chute while other employee was drilling from outside chute to install bolts when employee inside chute put hand in way of drill bit causing puncture wound
Employee is complaining about back pain. Cause of back pain unknown at this time.
Employee was shoveling and pushing wet limestone and felt pain in mid back. Went to md was diagnosed with a strain and given light duty restriction we did not have light duty work available with restriction.
Employee was bolting face area and fell over a gob pile and hit his knee on a rock. HE DID NOT START LOSING TIME UNTIL 7/27/09 DUE TO SURGERY
Mechanic was placing a 55gal drum of used absorbents & waste onto a pallet, he tipped the drum in order to ""roll"" it onto the pallet and strained his right shoulder.
The employee started missing work on 7/27/2009 due to walking up #4 entry on 3-section and twisted his knee on 6/17/2009.
EE was laying on rear tire to remove access panel. After removing bolts from panel, he jerked on the panel to remove it and heard a pop in his right elbow and felt pain.
IE was installing a section of C-channel when he noticed a pain in his abdomen/groin region. IE visited a local clinic later that day which diagnosed him with a swollen lymph node, NOT work-related. The following week, the IE's personal dr diagnosed him with a mild hernia. It was back and forth for awhile, trying to determine compensability. Finally determined compensable.
Employee was walking off of the catwalk at the truckwash when the bottom of walk broke. He stepped off of the side of the walk and onto a rock in turn, causing him to twist his ankle.
Employee was lifting a stone and felt pain in his back. Restricted duty assigned. Employee did not seek treatment until 6/18/09.
Employee alleges he was using a torch to cut steel and it went down into his glove causing 2nd degree burn to his left hand and wrist.
While loading blast holes employee slipped into wet and muddy hole and scraped his shin. The injury was not bleeding and the employee did not seek first aid treatment. Eight days later the employee went to doctor because of pain and swelling.
Employee was pulling a 30 hp pump in 2LW bleeder and tripped over a rock and fell to the footwall. EMPLOYEE DID NOT START LOSING TIME UNTIL 6/21/09.
The injured was roof bolting in Unit #5 in the 20+04 XC between Ent. 5 and 6. He was swinging his boom to install the right rib pin and caught his right foot with the drill pot, rolling it. The injured was diagnosed with several fractures requiring surgery to repair.
EE was working on #5 belt scraper. He was pulling out a belt pin, when he slipped and fell across the belt. He scraped and bruised his right rib and his right leg.
While removing tail pulley on a conveyor, it dropped pinching his left palm of hand. The laceration required 5 stitches and medical attention.
On 6/17/09 EE was bush hogging when he ran into the ditch causing the steering wheel to jerk - jarring his right shoulder. He continued to work until Dr scheduled surgery on 8/27/09.
Injury decided to move his cable anchor which was loacted on the corner inby section dumping point. A piece of brow fell and struck him on the right foot. Tests revealed three fractures to the right foot.
While scaling in preparation for mucking, Miner was testing the ground in the back when an area 11' long by 4 feet wide by 2 feet deep collapsed. Employee turned to get out of the way and fell, twisting his knee.
Injury used a power washer wand to wash grease off his boots. Water went inside his boot and burned his right ankle.
EE was unstopping a pipe in the plant. When he put his arm in the pipe, a rock broke loose and hit his right arm. It sprained his shoulder.
While removing a balance wheel with screw operated puller the belt in the puller broke, causing arm of puller to go airborne hitting employee in the mouth and damaging some teeth. He was sitting app. 12' away when struck.
Employee stated that he was carrying a 7 foot heitzman jack when his foot slipped on a piece of plastic pipe causing him to fall against a fiber crib. He said he felt pain in his right hip and lower back. He fell to his knees, then backward with the jack on top of him.
While using the impact gun to unscrew 3/4"" flat head bolt on a backhoe bucket, employee pushed on the flat head bolt with his left hand to prevent it from spinning. The flat head bolt began to spin and the edges of the bolt sliced into his left index finger causing a 1/4"" deep laceration that required 5 sutures.
The employee was standing on the top of the heavy media cyclone pump overflow box. During this time a fellow employee was tramming the overhead hoist across the plant. The frame of the overhead crane struck the victim, wedging him against a six inch plastic pipe. The six inch plastic pipe broke from its restraint, causing the victim to fall to the floor below.
EE was shoveling the belt when he tripped and was struck above the eye by a shovel held by a co-worker. EE alleges knee injury. The accident is being contested. (See Attachment)
EE was holding the 16"" slider tube with both hands while tow other EE's were sliding the 18"" vent tube over it in #5 entry, 3 North Left Section, when the 18"" tube caught his left thumb between the slider and vent tube. Ee was wearing gloves.
EE states he and another welder were making welding repairs to a dragline bucket and he became exposed to excess welding flash, in spite of wearing proper eye protection.
Employee doing cleanup of accumulated piles of material at #2 Cooler Fan area using skid steer. When he was exiting the skid steer, he stepped into bucket and felt pain in ankle. He commented that he had strained his ankle but did not report incident. Ankle started swelling overnight and incident was reported the following morning.
Loading truck, went into pile to get a scoop and hit a boulder that was mostly buried.
Employee was in the process of shoveling out the converyor chain on a highwall miner, when he lost his footing and fell.
Employee was changing short head on Miner, she cut the strap loose and the rail hit her on the lower leg (the rail sprung back due to the pressure being released from cutting the cable)

Employee was helping operator set up the drill rig to begin drilling. As the operator was lowering the kelly into the turntable the employee allowed his right index finger to be pinched between the kelly block and the turntable. Weather conditions were sunny, dry, and warm.
Injured was stacking 50# bags on a pallet. When he lifted one of the bags he said he felt pain in his left shoulder and neck. He said he has pain when he turns his head.
Four men were hanging miner, loader, bolter cables and 1 1/2"" hose, when he felt pain in left shoulder. He continued to work at his regular duties, but also continued to have pain. He eventually was taken off work for left shoulder surgery on 11/13/2009.
Individual was picking trash out of scoop bucket. He slipped and fell in scoop bucket hitting his right elbow and falling on his self rescuer on his back. Scoop bucket was wet and muddy. X-rays were negative and he returned on his next scheduled shift. He worked until July 14, 2009 when he went to the doctor. Doctor gave him off until July 27, 2009 for back pain.
Employee only worked for 3 shifts and stated that he got his foot tangled in the CM cable and twisted his knee. Operated was not aware of him being off due to the injury until 7/20/09.
Pushing hand truck and slipped on gravel.
EE was working on a coal hauler charger at the unit 5 coal hauler stations. EE had gone to the power center and was returning to the charger, he stepped in a rut and twisted his right knee. He continued to work until 7/6/2009, when he underwent arthiscopic surgery to repair.
The employee stated that he was lifting and moving a 58-HP de-watering pump and strained his lower back.
The subject slipped in the mud while retrieving a mill sample and pulled his groin muscle.
Employee claims that he was hauling material through a mandoor with 3 other employees when his left knee went out. He continued to experience pain and recently underwent surgery
The employee was putting pins in the #6 Preheater. While knocking the pins in he lost his balance and slipped. His foot went between the top rung of the ladder and the catwalk.
Employee was dismounting a loader to check his engine oil when his foot slipped causing him to fall approximately 6 feet down the boarding ladder, injuring his back and left forearm. Medical records were received on 6/30/09. Employee has been absent from work (10) days.
Employee was splicing a belt and caught his right thumb between the handle.
An employee sustained fractures to the little and ring finger on his right hand, when the hand was pinched between a rib roller and the protective cage on the miner. He was attempting to reposition the miner to free the ventilation tubing, when his right hand was pinched between a rib roller and the protective cage on the miner.
The injured was operating a roof bolting machine and was raising the drill head up to the drill steel, when he caught his right hand btween the drill head chuch and the drill steel. This resulted in a laceration, which required sutures.
Driver was getting loaded by excavator, operator dropped a rock and driver said seatbelt grabbed him and injured his back. Driver had previous injuries to his back, not work related. Had went to doctor two weeks prior.
Employee was doing clean-up and lifted an 80 pound piece of refractory to dispose of it when he strained his lower abdominal area causing a hernia.
Employee was building a brattice when he tripped over blocks causing him to fall on his left hip.
Contract truck driver stepped on a loose piece of decking and twisted his knee with a minor abrasion
EE received a laceration on the right ring finger that require sutures when he result between the bydoy seat and the driver seat of a 777 end dump truck to remove a trash bag that had been placed on the driver seat to hold keep the cloth seat clean.
Employee was found outside of scale trailer vomiting, disorented, and complaining of head pain- transported to E.R.. Several days after the E.R.and hospital stay the E. stated that the morning of the E.R. visit, he struck his head while preshifting his equipment. The E. stated that he was bent over checking the head of his vacuum truck, rose and hit the underside
Employee was clearing off a screen that was overloaded with material, when a rock fell off and struck his right foot.
While attempting to shut off the pressure line running from #177 shield to #166 shield, fellow EE bumped the pressure bleeder valve, sending high pressure emulsion through the 4' bleeder line. This caused the line to recoil and strike the injured EE in the face.
Injured party cut eye off of a jack on a 992 loader. The eye off the jack fell and struck another piece of the loader and bounced and landed on the top of his left foot buising it.
EE's of Bronco Drilling company failed to return to their rig site when returning from town. This was a frequently traveled road by all Bronco EE's to the rig. Investigation revealed that all 3 EE's were on their cell phones at the time and seat belts were not being worn. EMT attendants noted impairment of all patients.
He stepped from a wooden plank along the stage loader onto a crib that was placed under #1 shield and slipped, causing him to twist his right ankle, creating a right ankle sprain.

The TRS had lost pressure due to a leak. The employee was trying to find which hose was leaking by moving the hoses around. He told his partner to bump the lever, his hand was on the hose that was leaking (unknowingly), the hyd. fluid went through his glove and made a small puncture hole in his hand allowing hyd. fluid to enter his hand.
Reported: As he was attempting to install a nail in the mine roof using a hiiti gun, the roof popped, he jumped sideways resulting in pain to his left ankle.

EE was replacing a hydraulic hose to the conveyor chain motor on the feeder breaker. EE was standing on a 6"" crib block to reach the hose fitting that was 6' above the mine floor and approx 20"" from the frame of the feeder. As he was loosening the hose fitting with a wrench, he felt a sharp pain in his left adbomen area.
Employees foot was mashed between mantrip and coal rib resulting in a fracture to his right foot.
Employee stated that while dismounting a dozer, he slipped and fell from the dozer track to the ground. (started missing work on 6-13-09)
EE was trying to dislodge mantrip that was stuck in mud, when he fell injuring his back and left leg. He then missed work on the following day due to related back pain.
EE was installing roof bolts when a portion of the rib rolled out striking him on the back. Treated and released at hospital with scratches & bruises.
Employee was changing bit in roof bolt steel when gob fell and struck him in the head. This pushed his face down on tray of roof bolter. Fractured jaw and had to have sutures.
EE was loading holes, when he got back in power truck to sit down, his lower back started to hurt. He went off work on 6/22/09.
Employee was lifting cable over his head with 2 other employees. Cable was very tight, he felt a pop in left shoulder. Arm gave out and cable knocked him back. Employee worked until 7/23/09 and went off work on this date.
Cleaning scraper and stacker belt off, shovel got caught between scraper and belt line.
Employee was moving the wheel burro rock duster between #4 and #4 1/2 wall crosscut in #3 entry. E-20 beltline when she slipped on wet uneven bottom and felt pain in her left knee and left shoulder. She continued to work and began missing due to the knee injury on 6/23/09.
Tripped over floor mat while weighing truck at the counter
The man was cutting a seal off of a bucket, when the knife slipped and he cut his finger.
The employee was working on the Main West head roller out in the pit. When he was done, he stepped on the belt rail to get down. His foot slipped and he tried to catch himself by grabbing the coal shute, which caused him to strain his right shoulder. He continued to work until Monday, 6/29/09.
Employee observed spillage at a belt conveyor transfer point. Employee twisted his knee as he reached for the emergency stop cord on the plant feed belt conveyor. Employee apparently strained ligaments of right knee. As per doctor report, employee will be able to return to work on 6/15/09.
EE was struck by a piece of draw rock while bolting. It hit his left arm and shoulder. He suffered from cuts on his arm and soreness in his shoulder. No broken bones.

EE was driving unit #1199, a 190 ton end dump, across the shovel bench to get a load of rock. He drove through a dip in the road. He hit his knee on the dash. He went to the hospital ER, contusion to knee.
While roof bolting, his glove got caught on pinner steel and jerked shoulder. ** started missing on 09/28/09**
Employee was trying to take some bend out of 3/4"" conduit and was pulling on it with his right arm. The pipe twisted and pulled his arm causing pain in his right elbow.
EE was reaching for the chuck end of the jackleg when the leg extended out pushing the chuck end against the inside of his left foot pinning it against the rib causing a contusion.
While putting starter steel into drill head, piece of rock fell from roof, hitting him on left hand and broke his hand.
Employee was picking a 2 inch air pump out of the mud in 13H can line and felt a strain in his groin. ***Diagnosed with hernia and began missing time on 6/19/2009***
Employee states he is experiencing pain in his left shoulder which he feels results from heavy lifting during the longwall move. Claim is being filed to facilitate medical evaluation and final decision is pending on claim being work related.
Employee climbed onto a stopped conveyor belt without it being locked out. The belt started and he was caught by a material hopper, resulting in mulitiple injuries.
Working with stone and hurt back. (220091800025-Existing injury for the employee. He was able to work until 6/26/09. He has a tailbone injury that is going to require some off work time)
Employee was working on miner cat pads when a loose piece of rib peeled striking him on the left shoulder and lower back.
The employee was bent over when a piece of material hit him in the back (middle). The employee continued to work until 8/7/09 when we sent him to see a specialist. An MRI was done on 8/13/09. We did not get the results until 8/20/09. On 8/6/09 the employee started hurting - said he couldn't work.
Employee was moving a rock off of the shuttle car when he pulled a muscle in his lower back.
EE was assigned to clean up spillage in blending bin pump pit by using a long handled shovel to put mtl in 5 gal bucket. EE then carried up narrow stairway (12 steps)-EE did this task on/off over period of 9+days. Repetitiion of climbing stairs w/bucket resulted in EE suffering low left back & right knee pain.
Employee was helping anchor tailpiece and while holding one of the hydraulic jacks got his right hand caught between the jackbase and roof pad.
The worker has had a torn Meniscus of the left knee since Febuary 2008. On June 8, 2009 his knee became sore while performing his work duties. The worker on June 30, 2009 was scheduled for surgery to repair the torn Meniscus on July 9, 2009.
Employee was laying on his back and lifting a driveshaft into place. As employee attempted to lift the shaft he felt a severe pain in his lower back.
Employee was using the MRS Remote when he stepped on a crib block and strained his left knee. He continued to work until 7-13-09 when he missed work in preparation for surgery on 7-14-09.

Employee was getting loaded under the P&H shovel. The first bucket that was put into the truck had some oversized rock in it. When it hit the bed it jolted the operator of the haul truck hurting her lower back. This was on employees last shift for the week. She was scheduled back on June 12th employee did not make this shift.
Injured picked up a box of glue off back of bolter. As he urned to go to front of machine, he felt pain in his middle back. Pulled muscle in middle of back.
Employee states he strained his left forearm while he was unloading a ram from a truck into a box car. He did not request medical care until 08/04/2009 and missed his first scheduled work day on 08/07/2009.
Employee slipped while dragging a rock dust hose through a muddy area and fell striking his left shin on a rock. He also experienced pain in his lower to mid back.
He was doing permissability on the disconnects on the tight side of the stageloader. The rib rolled out and he tripped and fell striking his head against a tailpiece anchor abraision and cervical strain.
While installing supplemental bolts on the 2B longwall face, the EE was struck on the left foot by a piece of material, which fell from the top. The impact caused him to fall, striking his left shoulder.

EE was moving metal into a rack when he caught his left thumb between the piece of metal and the metal rack crushing the tip of his left thumb.
EE was feeding the clinker marginal hopper during the 3rd shift. He stepped down from the loader to check on the clinker marginal conveyor belt. There is a small ramp next to the hopper that leads to the clinker transport belt area, on his way down the ramp he slipped and fell, injuring his left knee.
EE was traveling in a mantrip to the section when the mantrip hit a piece of draw rock that was next to the track. It spun the rock around and it hit him on the right foot, it fractured his 2 small toes.
Ee was walking, stepped into a hole and turned his knee. Doctor said it was torn ligaments and that surgery would be needed.
The employee had on safety glasses and a chip flew into his eye. He was grinding something.

While driving a 10 wheel dump truck on a muddy road, injured slid sideways & struck sollid dry road causing the truck to turn over on it's side. The injured was wearing his seat belt, but hit the door with his shoulder & fractured it. ** The injured only works part time for the mine, two days a week.**
Employee stated that a co-worker had picked up a track rail with the forklift andhe rail slipped off the forks and fell on his right foot.
The man was closing the scrubber compartment door on the continuous miner, when he pinched his finger between the door and the machine.
The employee was moving a shuttle car out of his way. He placed his right hand on the canopy as the car moved. The canopy caught his fingers against the roof mashing them.
While hanging conduit for new conveyor EE hit elbow on one of the conduit hangers severly bruising elbow.
Employee was getting a 1 gal bottle of windshield washer fluid from foreman when he slipped and fell landing on the track of the d-9 dozer he was operating. He started loosing time on 6/8/09
Going up ramp to put bucket of sand in hopper. Raising bucket to get to right height to dump. Malfunction caused bucket to free drop. Operator stopped free-fall with control lever causing a sudden stop, causing operator to be thrown against seat belt violently causing whiplash to legs and back.
Employee placed hands on plate of adjustable leg to lift. He didn't use handle made for lifting. Leg came up and smashed thumb between plate and frame.
Reached in to remove a rock. Shirt sleeve got caught in the conveyor
The EE was cutting a 2"" X 4"" board with a curcular saw, when the saw kicked back and cut the EE on the right leg, requiring sutures.
Stepped down off scalper, stepped on loose rocks and twisted his left ankle.
Contractor's driver finished unloading glycerol into mine's storage tank. Proceeded to unhook hose from storage tank and due to pressure build up, hose blew off of connection causing glycerol liquid to splash into driver's right eye forcing safety goggles off driver's head. Grit was in the liquid and scratched right eye cornea. Right eye is healable.
Cable was slacked when tension caused cable to slip across spool causing right foot to be pinned between cable and spool.

The injured employee was putting guard back on the No. 1 conveyor belt tail pulley and dropped the guard striking his little finger (pinky finger) on his left hand.

WE APPEARED FOR MEDIATION WITH LEGAL COUNCIL. THE DATE REPORTED BY EE & HIS ATTORNEY IS JUNE 4, 2009 (?) FOR THE DATE OF INJURY. CURRENTLY THIS INCIDENT IS UNDER INVESTIGATION.

EE was running a 992 Cat front-end loader and was loading rock trucks. He was backing away from a truck with his boom in the air, he had not noticed that the keeper had come out of pin housing, which let the boom pin come out. The boom dropped and it shook the operator. Took two days off for soreness.
Employee was using a slate bar to free up a lump of coal that was stuck in the feeder area. The coal released suddenly and the slate bar struck the employee's left thumb, resulting in a laceration and fracture.
Oxygen line on torch burst open, caught fire, and blasted fire onto EE'S forearm. EE received 2nd Degree Burn.
The employee was lifting an acetylene tank and experienced a pain in the lower back. The employee started missing work on 7/14/09. He has been diagnosed with degenerative disc disease.
Driver was going to spoil dump and the driver hit a rough spot and it snatched him around. The driver was wearing his seatbelt.
Employee stated while performing center bolter duties, he was putting the head of a cable bolt into the bolt wrench when the bolter operator lowered the drill boom catching the victim's rt. foot between the drill pot and the mine bottom. This resulted in a fracture of the metatarsal bone just behind the metatarsal protector of his boot.
Employee was using a bladder pump to remove water from a bore hole when bladder ruptured forcing water & small rocks up the hole. The force of the water pushed up his safety glasses allowing material in his eyes.
Employee strained his left shoulder when his foot slipped while dismounting a D10 dozer. MRI results were not received until 6/19/09 and other medical records were delayed. This resulted in a reporting delay.
Was cutting small trees, slipped and cut himself with saw.

The miner was trying to pry a battery cover open with a hammer the lid slipped and struck small finger of left hand causing contusion and abrasion to the finger.
EE was walking in the #2 Entry, F-18 Section, when he stepped in a hole and felt a pain in his right knee.
EE was entering building, stepped down and slipped on wet bentonite, twisting left knee. EE continued to work and had no lost days of time until EE's surgery on 08/04/2009.
EE was washing the mast on the drill. He was walking along the mast washing it when he slipped. His legs slipped out from underneath him and as he fell, he sprayed himself with the power washer, which was equipped with a stripping nozzle. Took a chunk of meat out of his leg.
Employee was lifting the miner cable to hang in #1 Entry, E-21 Section, when his right foot slipped in coal and he felt pain in his right knee.

Employee was standing on the skid runner portion of a portable water pump, slipped and struck his leg on suction hose screen. He sustained a bone bruise on anterior edge of tibia of left leg.
As EE was tramming the shuttle car, the tram pedal stuck and the shuttle car hit the rib causing the operator to hit his head on the cab of the shuttle car resulting in the operator jamming his neck.
Injured was helping pull pipe into area behind the longwall. The pipe became hung. When freed the pipe sprang back hitting injured and knocking him down. This resulted in a back strain.
Working on forklift, laying on side. Strained neck while holding head up.
Upon an investigation, the employee was struck on the shoulder by a belt conveyor counter balance weight, causing him to twist his left ankle. The event has caused a lost time injury.
Individual intially reported injury to self at home. Later investigation revealed on 01/07/2010 that employee was injured at work. Employee injured back (3 crushed vertebrae) upon falling from bank/berm of silt pond.
Employee stepped on a small rock and rolled his left ankle.
DID NOT START LOSING TIME UNTIL 7/31/2009. While getting off Dozer, slipped, caught hand rail and pulled his shoulder. Surgery required.
Employee felt a sharp pain on right side of his groin area while loading posts on the scoop bucket. Original surgery postponed & employee missed no work. Employee will begin missing work for surgery 11/13/2009.
The employee was holding open the metal lid on a miner scrubber when the dropped the metal lid on his right thumb. He fractured his thumb in two places.
Lifting a hydraulic piston with a belt. The belt broke and the piston landed on the employee's right foot.
The employee was placing condensers in Furnace #9. When the condensers would no go in, the employee went to remove it. His left hand was holding the front of the condenser and his right hand was holding the rear, as the condenser was at the end of the retort, he started to lose grip and he went to catch it with his right hand. That is when he left a pain in his right arm.
EE was getting out of a utility loader (unit #1026 IT14) when he slipped. When he tried to catch himself, he jammed his thumb on his right hand (sprained right thumb).
Pulled Muscle in back according to Dr. EE squatted down put both hands underneath the cylinder and began lifting up the cylinder off the ground with the bottle. He reached for the tailgate of the truck and eased his behind on the ground
Employee was leaving a chute on the North Stacker, when he slipped and hit his left hand, causing a fracture.
While moving a DynaPac roller on the job site the operator lost control of the roller. The opertor removed the seat belt and jumped from the roller. The operator fractured her right leg when she hit the ground.
Employee was installing a guard on the 15B conveyor and the guard fell on his left foot, striking the top.

EE was on the powder truck and had the bay doors open trying to get the powder to fall (he has a stainless steel rod to knock the powder down). The wind caught the bay door and it fell, hitting his hand (contusion to right hand).

Employee was walking in mud, water and flyash behind the mule, watching the cable when he felt a burning pain in his left foot. His boot had a hole in it, causing a friction burn that now has a staph infection.

EE was standing on the counterweight of a CAT 992G loader talking to another employee when he slipped and fell approximately 5 feet to the ground.
Employee slipped on the muddy mine floor while setting a mine pump. Employee was twisting while moving a pump. This resulted in an injury to the right knee. Employee worked until 8/12/2009 when surgery was done to repair knee. Employee returned to work on 8/21/2009.
The subject was washing truck scales with high powered pressure washer, when he inadvertently struck the palm of his left hand, subject's hand is swollen and aching with a small laceration in the center.
Employee was cleaning up at the end of his shift. He was on the platform at the collection tank. He was lifting the lid on the tank with his left hand when he felt pain in his left shoulder area. On 6/29/09 he was diagnosed with a bicep tendon rupture. He was placed on restricted duty.
employee was opening a compartment door when suction inside the compartment pulled the door down.
Two EE's were cleaning and oiling bearing housing with bearing inside from vibrating screen. Bearing housing was set up on its side and oil caused bearing housing to slip and fall over. Bearing housing caught end of thumb on right hand of EE, crushed/cut end of thumb on right hand.
The injured was removing a trougher at the tail end of the weigh belt with a pry bar, his hand slipped into guard.
Employee pulled his torch hose from under a bushing and strained his back.
Initial accident reported to Supervisor on 6-25-09, EE thought the injury was very minor and did not want to go to the doctor. The injury was still bothering him and he went to doctor on 8-17-09. He was riding railcar to switch tracks, he jumped off before car came to a stop at switch, on landing he felt a sharp pain in the back of right foot and heel areas.
Employee had bent down to pick up a crib tie when a co-worker tripped and dropped the crib tie he was carrying on top of employee's right hand.
Employee claims his back started hurting after lifting form panels. Did not report in timely manner. Went to Dr and is off work from 6/25 on.
EE was assisting drill operator in loading new drill rods onto IR DM45E--EE had attached lifting device to rod & jib hoist but as rods were drawn up they were snagging on 4x4s they were laying on. As third rod was being drawn up, EE chose to assist by lifting trailing end over the snag spot--felt pull in groin.
While repairing conveyor belt on routine maintance, employee relocated belt position by hand and his finger got caught between v belt and sheave. He was immediately taken to Feneca Health Care District for immediate medical care.
EE stated that while he was roof bolting on the left side roof bolter(installing 8 foot cable bolts), he felt something pull in his right shoulder. This caused severe pain while attempting to move his arm in the upper position. He was taken to the hospital and x-rays were taken. He is scheduled for a MRI at a later date. The doctor believes he has a damaged rotator cuff
The miner was tightening the belt using a come-along (tug it). He felt pain in right hand. Pulled muscle & tendons.
EE was atempting to open a bottom dump railroad car into the track hopper. When the employee pulled on the handle to open it, she felt the doors did not open, so she decided to close the lever and try again. As she pushed on the lever to the close position, the handle snapped back suddenly catching her right ring finger.
While handling a hydraulic cylinder in a tight spot, person's finger got smashed
Employee was operating back hoe moving raw material in front of pit feeder for block wall placement when he felt tightness in his back from bouncing in the seat of the equipment while operating it.
As employee was removing guard from apron pan conveyor, the guard caught, causing him to lose his balance and take a couple of steps backwards. As he did so, he stepped in a pool of hot kiln feed.
EE was inserting a tip with a hammer into a scaling bar, when a metal piece chipped off striking him on the inside of his left forearm, causing a laceration/puncture wound that required stitches.
While attempting to lift access lid on a VSI crusher, employee strained muscles in his shoulder.
While walking on catwalk at end of day, employee slipped and fell on right side.
Employee was working inside #8 washer using an 8# sledge hammer to knock scale off rake plows. He slipped on wet floor and fell backwards, striking his neck on a rake plow. Employee was unconcious for 2 - 3 minutes. He was assisted from tank by another employee.
Employee was pulling on a charged hose and felt pain in his right hip. He was evaluated by a physician and was taken off of work on 6/18/09.
Employee was unlocking a 5th wheel on a lowboy tractor and trailer. It was stuck and had to pulled hard to unlock. The employee injured the rotator cuff in his left shoulder that will require surgery to repair. Surgery is scheduled for 6-24-09.
Employee slammed finger in cab door of haul truck.
Employee was closing a valve under a tank (pulling up on the handle) when his left shoulder popped and he hit his head on a brace that was overhead.
Employee was walking down a flight of stairs when he lost his footing and fell coming to rest on the landing below. He fell approximately 10 feet down the stairs.
Injury was walking at the surge bin and slipped, injuring his right knee.
At the start of shift, while riding on the mantrip rail into the portal, the front mantrip derailed. Employee stated that he was ok, but a little sore in the lower back area. He worked his scheduled shift on 6/14 & 6/15. On 6/16/09 he sought medical treatment from a chiroprator who took him off work.
Walking on toes of longwall face shields when foot slipped and fell between toes and relay bar. He suffered a fractured fibula.
Employee was knocking the dust off of a fan when the front guard fell off. His gloved hand hit the fan blades resulting in a fracture to his thumb. Lost time started on 6/18/09.
Employee was helping load forklift forks by hand into the back of a pickup. When they slid the fork in, the employee's left little finger was smashed, causing a compound fracture.
EMPLOYEE WAS WALKING FROM ONE SIDE OF PLANT TO THE OTHER TO ACCESS THE FEED TUNNEL. EMPLOYEE STEPPED FROM CEMENT GROUND TO DIRT GROUND, ROLLING RIGHT ANKLE AS HE STEPPED ON TO DIFFERENT SURFACE.
After blasting a round & barring down & then mucking out the round it was found that the lifters had not pulled. While cleaning out the holes to re-shoot them, a rock fell from the back or rib hitting the ground and rolling across the miner's foot breaking it.
The employee when prying with a slate bar to remove a bull gear at the no.3 belt head and strained his back.
EE was lifting crib ties over his head, when he felt something pop in his right shoulder. Restricted duty injury.
Employee was assisting with threading a drill steel and got his hand wrapped up in the steel.
Employee was cleaning up junk rollers along side the 5 ROM belt when he felt sharp pain in his right groin area. Employee had surgery 06/12/09. Lost time injury as of 06/12/09.
During maintenance procedures of screen machine the loader operator struck the elevating conveyor while miner was on the conveyor pinching him against the chute. The miner was given first aid, an ambulance was called and he was tranferred to Mayaguez Hospital where he was attended to and released that same day.
EE was checking a pump guage. He turned and stepped over a 3' line. IN doing so, his left foot got caught on the line, causing him to trip and land on his L ankle and breaking it.
Employee was working on top of five foot scaffling in the #2 inner seam slope. As he was getting down off the scaffling he fell on his back onto an I-beam that was laying on the mine floor causing an injury to his lower back. (Misc Mail 6/29/09 #3-this did not become a lost time accident until 6/22/09)
Driver was attempting to get back in his truck and stepped on guard rail on scales. His foot slipped off and he fell to the ground injuring ribs.
Employee was attempting to clean hardened material from the inside corners of the loader bucket on Unit L107 (Cat 976) when he felt a pain in his lower back.
On 06/05/09 employee was lifting & loading belt structure into scoop bucket when he felt pull in lower left abdomen. Employee continued working full duty until 07/28/09 when physican instructed him to work ""light duty"". Employee is scheduled for hernia surgery 08/04/09.
EE was helping with maintenance while the shovel was down. He went to pull the ladder up that was still latched (it has a sliding ladder) and he pulled a tendon in his left arm.
While the employee was on a ladder hanging a ventilation curtain, a piece of draw rock fell striking him and the ladder causing him to fall from the ladder. He suffered a laceration on his left arm and a contusion to his left hip.
The employee fell between his excavator and a retaining wall at the fueling area because of muddy conditions. He bruised his hip and knee and tore the muscle in his bicep.
The lid on the VSI was opened at the end of the shift to do maintenance. It was left open overnight to finish the next day. When they started changing lid liners, they didn't know the Hyd pressure had bled off but the lid was stuck in the raised position. EE stood on the lid & it dropped apprx 2 ft causing him to loose balance & fall off. He was not wearing fall protection.
Employee was handling a glass beaker. When moving beaker employee felt a sharp pain in wrist. Employee recently had surgery procedure on this wrist (not work related). It is assumed that the extension and rotation motion of the hand may have caused some injury to the previous surgergical site.
