Fuse on Number 1 hoist motor for the Production Hoist blew. Fuse had to be replaced. Had to evacuate the mine while the fuse was being replaced, because the Production Shaft is the secondary escapeway for the Mine.

A roof fall was found by pre-shift examiner in a four way instersection adjacent to # 12 & 13 1 west seals at 11:45am on 10/30/2009. Area had been supported with 6' full resin pins and had been truss bolted. No injury resulted. No plans to clean up the fall. Pending approval of a seal plan from MSHA, new seals will be built to replace those which are inaccessable.
The roof fall occurred in an intersection and went into x cuts both ways. Area was bolted with 42"" x 6' fully grouted bolts and a few cable bolts. Fall was approx. 6'-8' high - 70'-80' long - approx 18' wide. Area was cribbed and dangered off.
A roof fall occurred on the #3 belt flight, in the #5 entry. The fall was approximately 18,000' outby the working section. The fall measured 20' wide, 5' high, and 100' long. Person, vetilation, nor equipment were effected. This area will be rehabilitated.
An unintentional roof fall has occured at 19xc on the main east between the entries of 4-3. The fall was approximately 50 long by 17 feet wide and 8 feet in heigth. A scoop was covered up on the battery end of it.
Two employees were using the Deer Creek escapeway hoist when the motor tachometer malfunctioned, resulting in an overspeed fault, stopping the hoist. Due to the remote location of the shaft, the employees were caught on the hoist for approximately 45 minutes until the fault could be cleared from the surface control panel.
The mine encountered an unintentional roof fall on 10/29/2009 at 8:15 PM on the 2M.E. Travelroad between XC#2 and #3, the area was developed on 12/28/07 and bolted with 6' resin bolts. The fall measured 17' W X 20' L X 6' thick, the fall may be cleaned at a later date.
An unintentional roof fall was discovered at 9:47 am at brk 19 in the entry of the first right panel that was started at the #1 tail area for future section. The fall did not block the airway or escape way. The fall measured approx. 80' long x 20' x 8'.
A rock fall was found in the Southwest Panel #5, at BEP #5 in the number one entry. The fall was approximately 5' thick X 70' long X 20' wide. The fall did not block or hinder the ventilation.
We lost a power regulator at the main substation which killed all power to the mine. We started the generators and evacuated all personnel from the Underground. The power outage occurred at 0900 and all personnel were evacuated by 0945.
Discovered a roof fall in the HL1 panel at spad #3117. Fall is 4' thick x 20' wide x 25' long in the intersection and was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 2700' from the section and has been dangered off.
MSHA Inspector and Mine Examiner discovered a roof fall during a weekly exam of airways. Location of roof fall is 4 cross cuts inby, spad #25145 in adjacent entry. Estimated size of roof fall is 10' high, 20' wide and 140' long. This area was mined sometime in 12/2008. No injuries were reported as a result of the roof fall.
An unplanned roof fall occurred in the #4 entry at break #40 on the no.4 belt flight. The fall was 1 x-cut inby s.s.# 1536 the size was 20' wide X 40' long and app. 8' high. The area was dangered off and a plan submitted to rehabilitate the area. The fall was called in at 12:16 p.m.
Miami County Highway truck pulled up to stone pile behind Rock Industries Hauler truck. Rock Industries EE was unaware that the County truck pulled in behind him and backed into the passenger side of the County Truck. No injuries occurred. The County truck had cab damage to the passenger side.

Estimated size of roof fall is 10' high, 20' wide and 140' long. This area was mined sometime in 12/2008. No injuries were reported as a result of the roof fall.
Discovered a roof fall in the HL1 panel that fell from spad #3098 thru the break to spad #3102. Fall is 4-5' thick x 20' wide x 70' long in both intersections and the break. This area was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 2700' from the section and has been dangered off.

An unplanned rooffall occured in the #2 gas mine at spad #577 that measured 100' long x 19' wide x 10' high. No injuries were reported. The area was developed on April 10, 2008.
# 3 hoist tripped on low brake air pressure caused by a breaker problem on transformer. Breaker was replaced and hoist was back in service at 4:53am. There were still two escapeways throughout the incident.
A roof fall measuring 70'L X 20'W X 6'H occurred at spad # 8424 in the SE NO. 4 Panel approximately 4,503' outby 002 MMU (#1 Section). Area was bolted with 6' torque tension bolts.
IGNITION: The Continuous Miner was cutting bottom in No. 4 Face and had a methane ignition that lasted approximately 1 minute and was put out with a fire extinguisher and water
Roof fall occurred at the headgate area of the longwall 16-ft wide, 12 ft high, 12 ft long. Fall occurred in front of the longwall supports in an area of the roof where a sandstone roll was present. Fall was reported and investigated by MSHA and State roof control specialist. No injury or equipment damage to report from this occurrence.
A roof fall occurred on the #5 section outby the feeder in the #4 intersection at spad #49614. The fall was 8-10 feet high, 20' by 20'. The area was bolted with 6' torque tension bolts. and rope bolts. The fall will be cleaned up and a Jennmar Corp. 3 piece radius arch set system put in place.
A roof fall occurred in the return entry located at # 17 crosscut along the # 2 belt line No injuries from this fall. The fall measured about 18 ft wide 18 ft long and 6 ft high.
A roof fall was discovered by weekly examiner and Federal Mine Inspector at break outby spad # 2693 on B3 area was dangered off.
An unintentional roof fall was discovered I the intake air course outby 4200' of the 001 section in the #7 entry. Travel way is not blocked due to multiple entries. The fall was breakred off with cribs. Size of fall appr. 50'long x 19' wide x 4'7' thick, the fall consisted of moist tack sandy shale.
An unintentional roof fall occurred at 49 crosscut on the 5A travelway station 26+60 entry #5. The fall did not affect ventilation or cause personal injury. The fall did affect the secondary escape route which will have to be re-routed. The fall measured 20 feet x 25 feet x 6 feet. and was in a 4-way intersection.
A roof fall was discovered in the 5 North Left Retrun at 51 Crosscut, 69+18 to 68+13. Dimensions of the fall measured 12-15' high, 15.5' wide and 45' long.
The #1 elevator was out of service for a period of time exceeding 30 minutes due to the retirng cam cable being broken. No EE's were in elevator at the time. All necessary Government Agencies were notified according to the law. The retiring cam cable was replaced putting the elevator back into service at 5:05 p.m.
A non-injury roof fall occured during pillar recovery on the active 003MMU section, 5rt. pillar block, 100' inby S.S.# 7416, covering and entrapping the continuous mining machine. MSHA was notified and issued a 103(k) order which was modified to retrieve the mining machine. The 103(k) order was terminated at 19:21 hours to allow normal mining operations to resume.
A roof fall was discovered by weekly examiner and Federal Mine Inspector at 2 outby spad # 2851. Area was dangered off.
Roof fall on 14D Longwall, 63 Blk, #2 intersection. Approx 9' above coal seam.
A surface electrician was in the process of making his routine inspection of the Hallam Portal elevator when it stopped halfway up the shaft. Maintenance personnel reset controls to put the elevator back in service. There were no persons injured or equipment damaged as a result of this incident.
A roof fall occured in the 4g panel xc-17 #6 entry. The size of the fall was 50ft long, 19ft wide, and 8ft high.
Employees loosened the inspection door and material tricled out. The employees closed the door and beat on bottom of dust collector to get remaining coal to move through and out of system. When dust collector sounded empty, the employees checked with control. No more more material could be seen going through the conveyor system. When the door was opened, coal spilled out.
Weekly examiner discovered an unintentional roof fall on the 9E section in the #3 entry at the #6 crosscut. The fall measured approximately 40 feet long by 20 feet wide by 15 feet high.
The A-belt section was pillaring and the right side MRS jack weighted up at 3:00 pm. The crew decided to pull out of the entry with the equipment and the left MRS jack would not tram past a hanging rock and gob. The hanging rock fell from the mine roof around 4:30 pm and the crews used the miner to free the MRS jack.
While traveling in the second right area of the mine which is not active on 10/19/09, A foreman observed a roof fall adjacent to spad #448. The fall is approx. 45' long x 18' wide and 1-9' thick.No escapeways,ventilation,travelways were blocked. No injuries. Fall is now plotted on map.
An unintentional roof fall was found at 10:23 am on the old 1st right panel at spad # 651 in the #7 entry. The fall was approx 35'L x 18' W x 8'H.
Rain filled pit during the night. No employees at risk or damaged equipment.

# 4 hoist down due to electrical outage Friday 10/23/09 at 8:42 pm. Power restored & # 4 hoist running Saturday 10/24/09 at 9:00 pm.
An unintentional roof fall was found at 10:20 am on 10/23/09. The fall was one break inby spad # 544 in the intersection of #5 entry of the old 1st right section. Fall measured approx 25' L x 18' W x 8' H.
A trash can was parked in a track passway. Smoke was detected inby on the air flow. A fire was discovered at 11:50p.m. Fire was extingushed at 12:10a.m. on 10/23/09. MSHA Investigating.

The roof fell above anchorage in the face area of the No.1 entry on Section 6.
A roof fall approximately 50' x 18' wide x 6' high was found by the section foreman in the L-2 entry of the L-6 Rooms off of 2-Right at about 00:50 AM while making his rounds of the section. The area had been supported with 4-foot FG resin bolts. No equipment, personnel, or ventilation affected.
IGNITION: An employee was welding a bar to the rockdust pipe that was hanging down from the mine roof. He had been welding approximately 45 minutes and stopped to eat lunch. Another employee then burned a hole into the pipe and an ignition occurred from a methane bleeder around the pipe. The flame burned 10 to 15 seconds and was put out with 1 fire extinguisher.
A fall was discovered at 6:59 at spad # 406 on the old 1st right panel in the # 2 entry. The fall measured approx 20' x 20' x 8' high.
Our examiners found a roof fall during a weekly examination of a worked out area. The fall was located in the 1R/4L/ 4 Main North panel in entry #4 crosscut 5-6. The fall was approximately 50 feet long, 18 feet wide and 6 feet high. The fall was located in an entry adjacent to the examiners route of travel.
While making return air course weekly examination, discovered unintentional roof fall in the # 1 entry of 2nd N.W. Mains from SS # 3750 extending approx. 180 ft. inby in front of #2 & #3 seals of west butt.
A roof fall occurred in the South West Submain at station spad # 22+60. The roof fall was approximately 18 feet in length x 18 feet in width x 6 feet thick. The area was supported with 6 foot fully grouted roof bolts.
A roof fall measuring 30' L X 20' W X 8' H occurred in the #2 entry of the NO. 1 NW Mains off of NO. 2 SW Mains. Area was bolted with 5' glue bolts. Area was mined approximately 3-1-00.
A roof fall was discovered in the 2 north 8 worked out panel #1 entry crosscut 17. The fall measures approximately 20 feet long, 17 feet wide and 8 feet thick.
On 10/20/09 @ 19:23 an accident occurred on the main man hoist. The incident involved the south skip, when it was traveling to the dump pockets, failed to be stopped by the hoist operator. The skip hung in dump position in the head frame and severely damaged. Investigation revealed operator error was involved in the accident. Accident was reported to MSHA on 10/20/09.
Roof fall at No. 11 x-cut, No. 9 entry of 1 west mains. Fall was 20' W x 35' L x 3' - 5' thick. Cribbed and dangered off. Will not clean up. Area bolted with 5' torque tension 3/4"" roof bolts. Immediate roof was shale with laminated sandstone above it.
A roof fall occured at the 3a intake xc-91 the #4 entry. The size of the fall was 20ft long, 19ft wide, 6-7 ft high.
Highwall failure measuring 40'(H) x 5'(T) x 30'(W).
A methane gas ignition occurred on the 12 east cm section. No injuries or damaged to equipment occurred.
A fall of the roof occurred in the S.E.L. of main A at e-cut 83 in the supply rd. The fall measured 30' in length 15' wide by 5' thick. There were no personnel, equipment or ventilation affected.
Feedline came in contact with a roof bolt plate causing a fire that was extinguished within 15-20 minutes.
Employee was caught in an elevator for 55 minutes. No injury occurred. MSHA was notified immediately.
Production hoist tripped to manual at 7:10 pm. Electrician investigated the problem and could not get hoist reset. Mine was evacuated at 7:40 PM and MSHA was notified. Electrician replace SCR card in the west converter for the hoist motor. Hoist was returned to service at 1:20 AM. Case #1-68839331.
#2 hoist had electrical problems on the 250 volt side. Electrician was brought out of the mine to troubleshoot and repair. Hoist was back in service @5:45pm. There were still two escape ways during incident.
MPC on main shaft experienced arc flash, fire flared and was out within minutes, Mine was evacuated as precaution. Mine rescue team responded to confirm 1) that MPC was cool to touch and safe 2) mine ventilation was clear of gasses. MSHA inspector onsite released mine to return to work.
Roof fall in #8 return entry on panel C-2A 150' inby spad 5382 and 90' outby. Heinzmann jacks set and access to fall dangered off.
An unplanned roof fall occurred in the Coalbug Mine on #18 on #18 belt, #17 brk in the #2 entry that measured 20'long X 20'wide X 6'high. No injuries were reported, the area was developed on 7/13/09.
Attempting to put a natural gas warming flame on #1 kiln. There was a small accumulation of natural gas inside the kiln, which ignited, causing a loud noise.

Had an unintentional roof fall 40'x20'x8' in the return entry.
The scrap metal yard was discovered prior to shift change with smoke and spots of small flames at the Preparation Plant scrap yard. A water truck was summoned to the scene extinguishing the fire within (50) minutes. The water truck had to travel from the pit to the Preparation Plant allowing time to proceed beyond the (30) minutes threshold limit.
A roof fall occurred on the #13 belt line in the #4 entry approx. 2000' outby the face. The fall occurred in the intersection at SS6526 and measured 40'x30'x7'. The area was initially bolted with 48"" x 7/8"" resin grouted rods and 6' cable bolts C2 between rows.
Crew heard roof fall in adjacent room. Section foreman went to investigate and found fall in #2 room through #3 room. Fall was 18' high x 45' long x 5' high. Area was bolted with 4' resin bolts. Rock was laminate. This was in the 1 west butt section 95' inby spad # 731 in #2 room and 65' inby spad #728 in #3 room. Area was dangered off with cribs.
A face ignition occurred on the D mains section @ 4:20 AM. The power was de-energized and all neccessary government agencies were notified.
Break 51 on #2 belt, 1 break inby spad #309. The fall is from solid rib in #1 entry throught #2 intersection. The fall is approx. 8 feet high, 90 feet in length, and 18 feet wide.

There was an electrical fire that our tractor #4419 caught on fire. Nobody was injured.
A roof fall occurred in the #11 seam in the 2 Main North in the #6 entry in the intersection of crosscut #40. The fall measured approximately 13'x13'x1'-3'. The area was supported with 3' fully grouted bolts. A slip was determined to be the cause of the fall.
Roof fall was discovered at 01:20pm. Size of fall measured 18' wide X 100' long x 12' high. Fall does not affect ventilation or travel and will not be cleaned up.
Unintentional roof fall that occurred on South Mains crosscut #66 #6 entry. The fall measured 45'L x 18'W x 12'H.
An unplanned roof fall has occurred in the No 3 Entry at survey spad #142, the fall measures 20' in length, 20' wide, and 6' thick the fall does not affect travel or airways in this area.
Had a continuation of roof fall in #2 return airways that occurred 9/9/09. Area had already been cribbed and dangered off. Additional cribs and dangered off boards were installed to keep people out of affected area. Fall was discovered while making return airway.
The continuous miner made contact with old mine works from an unknown origin. Water came from the old mine to Cabin Hollow mine. All persons were safely evacuated from the mine to the surface. The water has been pumped and equipment removed from the mine.
A roof fall measuring approximately 5-10' x 18' x 70' occurred neat spad #2858 (no 5-6 entries)
At around 1:30 am the Hart street elevator failed to operate. The cause of the malfunction was determined to by a governor trip, which locked the brakes on the elevator. The governor was re-set and the elevator was returned to service.
A roof fall occurred on the #4 unit- #10 entry outby the tailpiece. The size of the fall was 50 ft long, 19 ft 3 inches wide, over 6 ft high.
A roof fall was discovered by mine examiner. Fall is located in the #5 entry adjacent to the 8-A belt, 3,500' from the active faces. Fall measured approx. 20'x20'x5'.
A roof fall was found on South B beltline inby xcut #23. Fall was approx. 35 feet long by 15 feet wide by 2 to 12 feet high. Area was bolted with Install II bolts and resin assisted. The supplemental support consisted of bacon straps, wire mesh screen, cribs and ""I"" beams. Fall was caused by deterioration of the mine roof through weathering.
On 10/10/09 there was a roof fall at XC 30 #3 entry Main West. There was no known indication prior to roof fall.

Coal mill air lock kicked out, stopping coal feed, causing the mill to heat leading to the possible ignition. No damage noted.
A rock fall measuring approximately 19.5 feet wide X 35-45 feet long X 54-62 inches high was found in face of the number 6 entry. Located approximately ninety feet inby spad number 4066. Fall was found at approximately 6:20 PM. The fall did not block the escape or air ways.
A ROOF FALL occurred in the last open crosscut of the No. 6 heading on the 001-0 section, in the interesection & fell right & left into the crosscuts. The fall was composed of laminated sandstone & was approximately 19' wide X 83' long X 10'+ high. The fall is located approximately 140 feet inby survey station No. 8340. The fall caught the continuous miner cable.
A reportable roof fall occurred in E1 Headgate, entry 1, between crosscut 25 and 26. The fall was approximately 32' long, 18' wide and 8' high.
A roof fall was discovered on this date. The fall measured 60' by 20' and was 6' in thickness. The fall was located on the 3-North Return in the #1 entry between crosscuts #51 and #52. The fall did not affect any persons or ventilation. The fall was dangered off.

A non-injury roof fall was found on the right return of the 002-MMU at survey spad # 507 at the #8 right crosscut and extending to the #4 entry to survey spad #504 above the anchorage of the 5 foot fully grouted resin rods used for primary sopport.
A roof fall was found in the old #3 entry neat the (A) set of seals. The roof fall was in the intersection starting at the 4th crosscut and proceeding toward the 5th crosscut from the old portals. The roof fall is approximately 60' long x 12' wide x 4 1/2' thick. Fall was found during the weekly exam. Fall shown on map as fall #2. Fall will not be cleaned up at this time.
This incident was a result of a storm that knocked power at the Hart Street portal. After power was restored the elevator would not operate. It required re-setting safety switches to resume operation.
A roof fall occured in the #11 seam in the 3 East Panel at crosscut #4 in the #2 entry. The fall measured approximately 18'x18'x7'. The area was supported with 4' fully grouted bolts and channel straps. A slip was determined to be the cause of the fall.
An unplanned roof fall occurred in the 12 I longwall tailgate. The fall measured approx. 16ft wide 8 feet high by 25 feet long. No injuries or equipment damage as a result of this incident.
This fall was found at 12:15pm on 10/7/09. The fall was 40'x30'x7' 90' outby SS 6591 in B northeast in the #4 enty. The area had been initially supported by 48"" cable bolts and supplemented with 8' cable bolts. The fall will be cleaned up.
The weekly examiner was conducting his examination and came across a roof fall located at north-east mains in the #1 return entry, spad # 4720. It fell 60 ft long and 7 ft high. It was dangered off. No one was hurt. This did not affect travel or ventilation control.
The time of this incident is unknown, it was discovered on the pre-shift exam. A roof fall was discovered on the 2B beltline at break #6 in an intersection. The fall measured 20' x 30' x 10' high. This beltline is abandoned, therefore, the fall will not be cleaned up. The area has been cribbed and timbered to insure the safety of surrounding areas.
During the weekly examination, a reportable roof fall was found in F-Seam, entry 7 at crosscut 78. The fall was approximately 18 feet wide, 13 feet long and 8 feet high.
Unintentioinal roof fall discovered this time and date. Fall located in 22 Zone, 13 to 14 block, no. 9 hdg. Fall measured approximately 70'L X 18'W and undetermined height. Area mined on or about 1/17/2007. Roof was previously supported with 6' full grout resin bolts. Overburden between 160' and 180'.
Following a prolonged shut down at plant operations, a quantity of coal began to smolder inside the coal storage silo. On startup of the coal mills, hot coal caused a fire on the belt conveyor. The fire was quickly extinguished within 30 minutes, but coal continued to smolder inside the pant leg of the silo.
An unintentional roof fall was found on mmu 002 working section in the intersection of number four entry one crosscut inby section loading point. The fall measured approximately 8-10' high, 20' length, 20' width. All approaches are cribbed and dangered off. The fall will not be cleaned up.
Unintentional roof fall discovered this time and date. Fall located in 22 zone, 11 block, No. 9 entry. Fall measured approximately 6'H X 18'W X 60'L. Area mined on or about 1/15/07. Roof previously supported with 6' full grouted resin bolts. Overburden between 160' and 180'.

Lower limit switch on Koepe man hoist failed. Problems incurred locating correct switch for replacement so mine was evacuated until problem could be corrected- no injuries; no other problems. Hoist back in service prior to following shift.
Weekly examiner found a roof fall at #31 crosscut in the #2 entry of the 3 Mains section. The fall measured approximately 8'L x 12'W x 12"" thick and did not fall above the anchorage.
A malfunction of the crabapple elevator occurred due to worn out contactor tips. The elevator was able to be operated by manual means, but could not be operated in automatic. This problem occurred throughout this shift and was completely corrected at the end of Midnight shift by replacing the worn contactor tips and replacing a relay.
Most likely the cause of the fire was leaking roof that caused an electrical malfunction leading to an electrical failure.The resulting fire resulted in a loss of the building. The building will be replaced and that would elimate any roof structural concerns and any possible electrical malfunction that would have led to the cause of the fire.
IGNITION: The Miner was cleaning up the face of No. 4 Entry on No. 5 Section and as he was cutting bottom, an ignition occurred on the curtain line side (right side) of the entry. The ignition only lasted a few seconds and it put itself out. No fire extinguishers or water was used.
An unplanned roof fall occurred on 2 Main North, #5 Entry at Crosscut 28. The fall was apparently the result of deterioration over time allowing the roof to fall above anchorage. The fall is approximately 35 feet by 30 feet by 7 feet high.
Contractor dry had significant damage to main structure and contests. No injuries were sustained in this incident. The fire originated to a fixed Wall heater mounted on the North West corner of the structure. These findings were determined by the Nevada State Fire Marshall.
An unintentional roof fall occurred due to the roof strata broke at and above the anchor point. Once the fall occurred it exposed a slip running parallel with the entry. All approaches will be reinforced and the fall area will be cleaned up and supported. The area was supported with 5 foot conventional roof bolts and 6 foot double lock bolts. The fall measured 30'x25'x6'.
The continuous miner operator alleged that he thought he saw a flash while cutting down coal between shuttle cars in the 8 right crosscut. There were no injuries and there was no damage to equipment. The immediate investigation after this allged event failed to uncover any direct evidence that an ignition of gas or dust occurred at all.
At approx. 6:30a.m. a fall above anchorage was discovered at break #44 near spad 1108 and 1109. The fall was approx. 40' long x 11' high x 18' wide. No roadway, airway or escape ways were blocked. No injuries occurred.
An unintentional roof fall was discovered in the #2 return 8 breaks outby the #3 set of seals. Size estimated to be 20'W x 6'H x 20'L. Area had been supported with 6' fully grouted resin bolts. No injuries, ventilation not interupted, no equipment caught. Fall will not be cleaned up. MINE has been in non-producing status since Aug 1, 2009.
A roof fall was discovered in the 3 North 4 Mains, 78 crosscut, #2 entry. The fall measures approximately 30 feet long, 17 feet wide, and 7 feet thick.
The Hart street elevator was going out on nuisance trips on overspeed. The investigation revealed that there was some mud build up on sheave wheels which increased the sheave wheel size enough to create the trip faults. The elevator was operational but it was decided to shut it down to clean the sheave wheels.
Kuhntown Elevator went down at 11:50 AM, no persons on board. Doors would not open at the top of the shaft. Door switch and slow down switch were replaced by Shindler. Elevator was back in service at 2:00 PM.
VICTIM WAS ON A BELT CONVEYOR TO CUT OFF A LOOSE PIECE OF BELT. HE FELL APPROXIMATELY 20 FEET TO THE GROUND BELOW. HE DIED ON JANUARY 11, 2011. HARD FROST THE MORNING OF THE ACCIDENT. FAILED TO WEAR COMPANY SUPPLIED SAFETY HARNESS.
Truck traveling from mine site. The victim had truck in high gear, truck was out of control, he put truck in ditch and jumped out. Victim received fatal injury to head.
The employee was assisting in moving the sample splitter table using a plastic pallet. It got stuck in the doorway. The employee pushed on the table legs, and his right hand got caught between the metal plate on the table leg and the door frame, causing an amputation to the distal phalanx of the ring finger and a laceration to the index finger.
While shoveling, the employee aggravated and enflamed a non work related pre-existing hernia.
Employee felt pain in his stomach and groin area as he attempted to lift a drill head in 4 south left section. Continued to work. *** Diagnosed with Hernia 10/28/2009.
Victim was tramming a twin boom roof bolter out of the #1 face. The roof bolter struck the rib pinning his right middle finger between the frame of the machine and the canopy post resulting in loss of the end of his middle finger to the second joint. Due to the mine down for issues with seals he has not missed any scheduled work.
While loading a fusion machine into the bed of a pickup truck, EE was lowering the handle of the machine, causing it to pinch his left middle finger. The injury to his finger required part of the distal phalange to be amputated. This accident was not received at Marfork until 10-23-2009
Lid covering pick breaker chain on feeder was raised up and braced so work could be done, when brace slipped letting lid fall suddenly hitting EE on finger of left hand.
Employee was lowering canopy on bolter and had his right hand on tray and brought canopy down on his finger, crushing the ring finger just below the nail on the right hand. Tip of finger was amputated below nail of right hand at the knuckle.
Employee was laying down some metal shelving on a push cart when his left ring and pinky finger were caught in a pinch point causing the ring finger to be lacerated and the pinky finger to lacerated and broken. Employee lost approximately 25% of the distal bone of the pinky.
Miner was doing maintenance on 3' Cone/Crusher. Had removed head and was replacing it and lowered head down into bowl assembly. The head was not down on one side and the chains had been released, he stuck the two fingers of left hand into 1/2"" gap, the head then released crushing the two fingers to the first joint.
crew of 4 men were turning over Dosco beams that were stacked upside down, EE did not let go as others were dropping the beam and the tip of his right middle finger was caught between the beam s as he pulled away. the end of his finger was severed and remained in his glove, no bone fragments were in the glove.
Employee stated he was helping move and hang the Cont. Miner cable when his shoulder which had been hurting started hurting worse. On the night of 12/06/2009 he left the mine with his shoulder hurting again and was taken off work on 12/10/09. MRI that was done showed Tendinosis.
Employee was lifting and removing empty 55 gallon drum barrels, approx weight of barrels were 6 to 7 pounds. While employee lifted the barrel approx 1 ft. off the ground and twisted around to lay the barrel back down felt a pop sensation in his lower back.
Closing Anfo Trailer Door walking backwards tripped and fell over ground cable backwards hitting back on a pickup.
Slipped while dismounting front-end loader causing a wrist sprain when he caught himself from falling. NOTE: This claim was originally contested. It was ruled a workplace accident by Ohio BWC on January 25, 2010.
While pushing a 16' cable bolt into the top with a gopher drill, the bolt slipped off causing the drill to rise quickly. The employee's hand was in a pinch point which caused multiple fractures and two lacerations to his right middle finger.
EE was building stopping and a block fell from top of stopping and fell on EE's left hand.

Shear operator cutting bottom going to the tail at 140 shield, cutting through a clay vein. Rock came off drum striking his rt. hand middle finger. His hand was resting on handrail when rock hit him. Cause of this injury was habits, compliance with rules/regs was not a factor,ppe was not a factor, miner profiency could be a factor.
The employee was removing an engine panel from a rubbertired front end loader when the panel slipped from his grasp. The employee tried to keep the panel from falling injuring his back.
Employee was climbing down the ladder of a 980 G loader. When he slipped and fell to the right on to the rear tire thru to the concrete floor in the surface shop.
While tightening a lugnut on a skidsteer, a bolt broke and EE fell backward twisting his ankle. He reported a sprained ankle. We administered first aid with an ice pack and bandage and he returned to work. Over the weekend it was hurting and he went to the doctor and had an xray and was told that the bone was cracked and would require a cast and 4-5 weeks off work.
Employee was standing on top of the 5' by 14' screen deck, utilizing a safety belt & lanyard. A moment later EE allegedly slipped, fell from screen deck & allegedly struck his back. This incident is being reported only because of Part 50 requirements, as the operator strongly suspects fraudulent behavior.
Employee states he was partially lifting mud flaps and turning when he felt a pain in his low back and left hip area. Employee stated at clinic visit on 11/2/09 this is due to 12/3/08.

Injured employee was walking the roadways prior to the start of his shift when a piece of draw rock approx. 26""x48""x1-3"" fell from between the roof bolts and struck the left top of his head and continued down the left side of his torso. He was airlifted to the ER and admitted to the ICU. ER Diagnosis: 4 Fractured vertebrae, small bleed on the brain and injuries to his left ribs.
EE was standing on the track of the D9G, removing a snow tarp. EE slipped and fell off the track landing on his right side onto the ground. EE felt pain in his right arm. EE went to the ER at Holy Rosary. X-rays were taken and determined he had a crack in the bone of his upper arm.
Employee was trying to get a fan hub off of a fan motor with a hammer and punch. When he was striking punch with hammer a piece of hub came off and struck him in the hand.
Employee was in the coal loadout tunnel watching the belts while the train was being loaded. He noticed a piece of coal was stuck in the skirt rubber on the south side of the belt (main). He removed the piece of coal and the conveyor belt caught his right hand and arm

EE injured his back on 06/09/1998: he had surgeries 2001 and 2002. There has been no new injury but he has experienced intermittent, chronic discomfort since that time. On 10/28/09 he requested medical care due to his worsening condition. He went in for surgery on 03/01/2010.
Employee was examining the CO monitor areas and the conveyor tail area. He grabbed a belt hanger chain and stepped up on the frame to look at the monitor. When he did this his foot slipped and he twisted his knee. He started missing work on 11/04/2009.

EE had exited cab of 45 ton mobile crane & was walking on deck to outrigger to step down to the ground. He allegedly saw a spot of grease on the motor compartment & bent down to wipe if off. As he raised up he hit his head on the jib section which was folded up alongside of the boom. EE staggered & fell off the mobile crane to the ground below ( six foot fall).
Employee was cutting a cable when the knife slipped and cut right upper leg. The laceration required 8 stitches.
Employee had proceeded off the job site when the brakes failed in his personal vehicle. The employee then attempted to maneuver the vehicle into a ditch to reach a stop, which resulted in the vehicle flipping several times. The employee sustained fractured ribs and vertebrae as a result of this incident.
Alternator failed, causing battery to overheat and boil dry. A spark was somehow created during the jump starting process, igniting the gas and resulting in the explosion of the battery.
After cutting off some old rusty bolts off of a valve and using a hammer to knock them out of the flange, EE received a piece of steel in his left hand. He had the steel removed from his hand on Tuesday the 3rd and started missing time on November 4th.
EE was assisting the mechanic put a motor on the highwall miner using a porta-power when a metal brace broke loose and struck him on the right shoulder. EE continued to work for several hours before seeking medical attention.
EE trammed minitrac in front of feeder and into crosscut, cal hauler padded under curtain while ringing bell. Minitrac & coal hauler contacted on another neither piece of equipment was damaged and pie pans did not fall off of forks.
Employee was setting timbers on 2D beltline when a piece of draw rock fell and struck him on the head.
EE was in the shop welding. EE put his gloves on when he felt a sting on his left hand. EE's hand turned red and swollen. EE went home and returned back to work with a bad red streak running up from his hand up to almost his arm pit.
The employee stated that he and another employee were making a splice on # 4 continuous miner cable. As IE was making the splice, a strand of the copper ground wire punctured the inside of his right thumb, at the first joint. Two days later the wound has become infected and he has had two surgeries to help remove the infection in his thumb.
EE stated he was tramming the center bolter out of the way. While doing this the wheel of the bolter hit a hole causing the bolter to compress him against the rib. EE was taken to a med.fac.and found to have contusions to his left hip and leg. Further evaluation on 10/2/09 it was found he has a left hip fracture and is now being reported.
Injury was helping install ring in #2 cooler and pulled his back while holding ring up in cooler.
Employee was standing behind a 3-wheel ride and was told that it was going to be moved. The ride moved in the wrong direction and caught the employee between the ride and feeder.
Employee slipped on the steps leading out of the hose trailer and fell hitting on his right knee. Fell approximately four feet.
While loading supply on bolter, Injury turn to pick up a stack of pizza pans and felt pain in his back.

STEPPED BACK INTO A SMALL HOLE WHILE FUSING PIPES TOGETHER - SPRAINED ANKLE
While in the process of installing additional roof support, employee's head and neck was struck by falling material, causing contusions and abrasions.
Employee was cleaning coal with a small loader and the coal in the pit broke causing the small loader to land on its side.
Employee alleges he was loosening bolts on a blowbar with a impact gun and the bolts came loose pinning his hand between the crusher and impact gun bruising his left hand. *Late reporting due to employee was just scheduled for surgery on 12-18-09 and will start loosing time then.
Employee was setting up to replace removable floor in transformer room in Dragline. Employee fell through the floor opening when trying to reach the shackle that was located above the floor opening breaking his wrist.
EE injured was bending a 8 foot roof bolt.The bolt slipped and injured strained his back.
Employee states he was in the borehole tank room and a pipe on the rock dust tank came loose, filling the air with rock dust. As he exited the room to get out of the rock dust, he walked into the vent air stream, which pushed him against the rib and bruised his left shoulder. Employee was placed on light duty.
Rock from working face rolled onto bucket while mucking pile causing loader to jolt. Employee complained of pain in back. Dr put him off work as of 3/11/10.

Employee was helping to tack on steel bars to a steel plate. Employee was not wearing adequate eye protection and suffered a flash burn to the eyes.
BHDF temporary employee was stacking bags of barite on a pallet in the bagging area located at the Mill for approximately 30 minutes when the employee noticed that his back was hurting. The employee kept on stacking bags of barite for an additional 30 minutes and noticed that his back was still hurting and then notified his supervisor.
The employee stated that he was walking on a barge when the barge cable recoiled and hit him under the right arm, knocking him into the loaded barge. The employee did not begin missing work for this injury until 1/14/2010.
The employee alleges that while unloading material in the hauling truck in the crusher primary area he injured his left leg, left shoulder and left arm and elbow.
Employe was trying to pull scubber door shut by pulling on chain attached to door. He grasped the chain with both hands to pull door shut. He felt pain in his stomach and belly button as he pulled the door shut.
Fell while cleaning a conveyer belt while using a hose to clean off the equipment. An employee had fed too much hose to claimant which threw him off balance. Claimant hit his head even though he had hard hat on.
Employee was attemting to close the door of his mucker when he felt a pain in his back.

While raising the personal canopy, it swung and a rock fell, hitting the left thumb.
Employees were installing 4"" pipe in floor drain between 3rd & 4th floor. While tack welding a 4"" 90 degree elbow to a 4"" pipe the pipe slid and the tacks broke and 4"" elbow fell striking injured on his hard hat and left shoulder.
Pulling miner cable out of perimeters, there was a wind row of gob, cable got tight and came over wind row and struck employee on the left ankle.
Employee was running tram when he stoped to set timbers. He twisted his left knee while exiting out of tram. Employee continued to work his regular job duties until surgery. Began losing time 02/08/2010.
Working in trench drain, stepped on rock and twisted ankle. Did not find out ankle was broken until 10/30/09 and sought treatment.
The employee's left foot was caught by the left side digging arm of the loader as he started to cross the coal pile.
EE stated that he was walking from power center to the face area, stepped on the ledge of a hole. His foot slipped in the hole suddenly causing him to jerk around side ways, hurting his back.
Employee was putting up belt hangers with bolter, hit rotation and loose clothing became caught on drill steel wrapping hand and arm up in steel.
Employee was taking a cover off of a shuttle car with a socket and ratchet. While loosening the bolts the socket broke causing him to fall and land on his right shoulder.
Drove through fly pad & leg got caught & bent back and broke them.
EE was using the retractable hose assembly to add engine oil to a pan. He pulled on the hose and the hose retracted or tried to retract. The EE reacted & tried to gain a better grip on the hose when he felt a sharp pain in the stomach area/upper abdominal area. EE continued to work his regular job until 10/30/09 when the doctor removed him from work activities.
Employee was using an air die grinder when it slipped from his hand and he tried to catch it. The grinder was still running and it cut his left thumb requiring 6 stitches.
While installing belt structure, the rib fell, about 12' long, 3 to 7 inches wide and 10 inches thick at the thickest point, hitting his hat and partially covering him with rock.
While pushing coal in stock-pile he stated he backed into the highwall injuring his lower back. IME report 12-23-2009 states that the lower back complaints are not causally related to the injury date,but were preexisting and naturally occurring such as a degenerative change or preexisting conditions.
Employee was stooping over D11R roller frame, cutting pins out of major boggle. He stood up and felt severe pain in lower back. Employee began missing work on 10/26/2009.
Stone struck worker's thumb causing him to lose thumb nail.
Employee was inside the framework of #2 surge conveyor preparing to cut an roller bracket off with a oxygen/acetylene torch. When employee lit the torch, he heard a gurgling sound. Employee attempted to the gas and air off. before he could get it off, the oxygen hose blew into causing 1st degree burn to the employee.
Was walking beside the power center and stepped on a rock. This resulted in a twisted ankle.
EE was in the process of installing a 72"" roof bolt. He was pushing the bolt in the hole when he accidentally hit the wrong lever and the bolt spun around, striking the ee on his left arm.
EE lifting steel plates/parts when he felt a hot burning pain in left inguinal area. Over the next few days, the pain increased and the employee reported it and sought medical attention. Employee underwent a layoff (planned production shutdown) during this time and was not eligible to work until 1/4/10, when recalled. and he was unable to due to medical restrictions.
Mechanic was connecting a conveyor to a screener. He was on the conveyor when it started up and he fell off.
Operator went across hump going to miner got head into top and shuttle car
EE was removing bolts to an inspection door cover. As he removed the last bolt, the cover swung down and pinched his left hand between the cover and the lift. Neither ee nor his assistant had secured the cover while removing the bolts. EE received ""boxer's fracture"" to left hand.
Employee was loading scrap iron into a loader bucket, felt pain in abdomen. Doctor determined it was just a strain and released him back to work on the 26th.
EE was using a slate bar to move a 60 lb. track rail that was loaded on a flat car, when it suddenly rolled causing his left hand to become pinched between the roof and the bar, fracturing his left little finger.
The bargeloader had climbed from top covers of barge down to the bow deck of barge using the attached three-step ladder. When he turned to walk toward the starboard side, he tripped on the lid of the bow wing tank causing him to fall, falling on his right shoulder.
EE was exiting the plant control house and slipped on the steps hitting his right knee and lower back.
While walking around the hi-lift, the employee stepped on a rock and twisted his ankle.
The employee was prying on a rock with the hi-lift when the bucket slipped off the rock jarring his back.
Employee was removing crib block from in front of #11 Voest Alpine Miner with a co-worker in the 3 North Right Section. Both employees picked up a crib block to toss aside when the crib blocks collided catching the tip of injured's right middle finger between the two crib blocks. Employee sustained a fracture to the right middle finger. Employee was wearing Meta Miner gloves.
On 10-21-09 EE was operating the maintenance truck boom/winch by remote while in the process of pulling belt through the highwall miner add-car frame. Co-worker was watching for hangups at the belt roll end. The steel belt roller that was used for the cable upward pull dislodged & struck him in the face. First Aid was administered and employee was transported to CAMC for treatment.
Block fell back. EE landed on knees.
Employee was lowering a chute with a hand crank device. He lost control of the crank sending it into a freespinning motion contacting his face. This caused a broken nose, fractures to bones in the face, and lacerations.
When exiting S&S unihauler to help continuous miner operator change place (heading) EE felt a pain in lower back. EE did not report injury until 10/26/09 at 9:30am.
While walking belt during examination, her boot got caught underneath rock, causing her to fall on knees. ** started missing 10/22/09**
Tightening bolts under screen when wrench slipped off and hit back of right hand on piece of metal. Sprained back of hand and broke 3rd finger.
While taking the rub rail off of a miner, the rail fell striking the employee in the shin.
The employee stated he was dismounting #766 diesel mantrip and twisted his right knee. The event resulted in a lost time injury tear of the medial cartilage/sprain.
3 employees were performing welding activities at the same time and in the process one employee was exposed to an arc flash from another welder resulting in temporary pain in the eyes. Employee sought medical attention at an eye doctor and was given one day off of work.
After extracting a hammer bit from crusher 2 with co worker, both EEs carried it down the stairs into a truck. While walking to the truck, injured stepped into a muddy area with surge rocks and twisted his left ankle.
While moving (tearing down & setting up) crushing plant, using blocks to lift crusher, it slipped catching his right hand, injuring thumb and parts of pointer finger.
Employee was injured when a pressure hose blew, striking his hard hat, causing him to spin aound and twist his left knee. Became a lost time injury 3/18/10 due to surgery.
EE was under a dozer. He was trying to put a pin back in an equalizer bar which the pin had come out, on the track frame side the equalizer bar dropped about 2 "". EE moved out of the way and hit his head on something under the dozer maybe a bolt 1 1/2"" laceration requiring 8 staples.
EE felt sick with back / side hurting and decided to go to the doctor. Did not inform the Foreman that he had an accident. Foreman thought he was sick.
Dismounting from the 988 loader, he missed footing on last step, did not use 3 point of contact.
EE was using a 10Lb. sledge to knock a corner protector off a loader bucket, when a piece of steel flew off the protector and hit his right leg. A doctor had to remove it.
EE's left foot was struck by a shield pontoon resulting in 4 toes being fractured.
Employee was trying to get a headache ball loose that had caught on the boom, when the ball fell, catching the employees arm between the ball and a piece of steel.
Miner was sliding shoot back on wash plant with pry bar.
Bottom deck sub frame sections were being removed from under a inclined screen. Injury was on the work platform(downhill side)when a section that was having to be ""pried down"" started a 2' ""free slide"" after coming into a ""loose"" fit area, He slipped while moving away from the section & fell backward onto the same work platform. *No safety chain was utilized.
Grate on catwalk attached to conveyor belt was lying on top of vertical channel iron. Vibration due to conveyor moving appeared to move material off channel. Employee stepped on it and the grate fell through.
EE was directing a scoop into the x-cut to move cement blocks out of the walkway. A longwall roof support can fell striking the EE and landed on his left leg. EE was treated and received multiple bone fractures to the left leg and a broken ankle.
EE was on his way to the outside, when a rock fell from roof, striking his left knee.
Employee was walking and stepped on a rock or lump of coal and said he injured his left foot. He walked off section by self and said he didn't want no help. After he got outside he walked in bath house, took his boots off. Didn't want no help.
EE was descending down a fixed ladder on a loader. The EE lost his grip on the ladder and fell to the ground. The EE injured his left foot as a result of the fall. Medical attention was sought and the EE was diagnosed with a fracture to his foot.
EE was laying decking and stepped beyond caution tape onto unsecured decking. The decking moved and he fell through the decking.
Injured was taken off work when it was found he had a rotator cuff tear. This condition is from trauma while performing the duties of a coal miner. Surgery is required to correct it.
He was installing roof bolts and pulled a muscle in his chest.

While pulling the MBC ahead during a move, a rock came out of the rib of a high spot striking employee on the foot.
The employee was standing on top of a loader with a rope in his hand that was tied to a drive line underneath the loader. As he was pulling on the rope he felt pain in his back. He continued to work and on 11-30-09 he was taken off work to start 2-3 weeks of therapy and is still missing work.
EE was helping secure a retaining fence on mt. face. He was working on a bench below workers located on top bench that were securing the same retaining fence. Debris was dislodged and fell from the top bench striking ee on his right shoulder/shoulder blade.
While setting a shield on the 7A long wall set up face, the injured activated the relay bar function in order to align the horizontal pin. This action caused unexpected movement of the shield pontoon which cam into contact with the employee's right foot causing multiple fractures.
EE was taking down belt hardware on X cut 5 CU 4 belt, EE was taking out rails from inside of belt hangers one slid out and landed on top of her left foot. Bruise to top of EE's foot.
EE was putting up rib bolt; suction hose got damaged; took off glove reached down with his left hand to fix hose and a piece of metal poked thru top of left thumb.
EE developed pain in his knee while crawling through the section. The EE worked until 11/02/09 when he reported off.
Injured, along with 2 other employees, were trying to push and bend belting to fit in back of scalping screen. Injured started having pain. Injured did not start missing work until 2/12/10.
Employees were attempting to pull direct burial wires thru conduit when employee lost his footing and fell in trench.
EE was walking to work area on flat ground and slipped and fell. He landed on his right hand and was complaining of pain in his thumb.
While pulling a length of 3/4"" hose, across the ground, the employee lost his balance. He raised his right arm to steady himself and partially dislocated his shoulder. The employee's right hand or arm never came into contact with anything.
Employee was standing at the side control station of the #6009 Roof Bolter when he was struck on the top of the head by a piece of falling roof rock. The rock knocked him to the ground and part of it fell on his midsection. The rock measured 4' long x 2' wide x 1"" thick.
The worker was removing a broken torque shaft from the tail side cutter motor. The shaft was stuck in position and the ee stated that he gave a hard pull and felt something give in his shoulder.
Employee was changing the direction of the diamond wire saw, which was stuck in the stone. He was pulling on it w/a rope, which separated and employee fell backwards.
Employee was operating a cutting torch. A small, hot piece of slag fell into the employee's right boot, causing a small burn on his right calf. Employee ceased operation, removed the piece of slag from boot, observed the small burn, and stated he was okay and did not seek or ask for first aid or assistance. Employee resumed operating the cutting torch.
The employee alleged he was dismounting scoop and twisted his back. He later went to the ER and stated he was lifting on job. The employee has started to miss work, and is seeking medical testing for any possible injury from event.
Employee stated that while he was unloading posts off of a Jeep, he had hold of one post and was pulling it off of the Jeep when the end of the post hit the end of another post and his right ring finger was caught between the posts.
Employee mis-stepped on a ladder on 10/16/09. Employee did not go to have a medical evaluation/examination until 10/26/09 at which point event qualified for report. Employee twisted left knee on ladder. Received cortisone shot.
Employee was making an on-shift examination when he slipped off a 1 1/2 foot ledge and strained lower back.
Employee was on a ladder hanging curtain. The ladder tilted causing employee to fall. Employee finished shift.
Employee was walking uphill with a hose and his knee started to hurt. Was seen in ER and was taken off work for a knee injury.
Injured employee was riding the mantrip to the surface when the mantrip derailed. Employee struck against the inside of the mantrip injuring his left ribs causing bruising and a hematoma on his left rib cage. He went to the ER later that evening and the X-Rays were negative for fractures. He was excused from work until further notice.
Injured was taken off work when it was found he had a hernia. This condition is from trauma while performing the duties of a coal miner. Surgery is required to correct it.
Employee was attempting to climb onto the top of a screen deck when his hard hat struck an overhead beam causing him to lose his balance and fall onto the screen deck.
Employee stated he was resting with his foot up on the bumper of the scoop. A piece of roof rock fell and struck him on the head and knee, broke and fell to the ground. He continued to work and then had a MRI done. His doctor took him off work on 12/16/2009. MRI showed a prior ACL surgery and degeneration throughout the meniscus.
Employee experienced a ""pop"" in his right knee while helping to move crib block. Continued his regular job until surgery on 1/18/2010.
Employee was operating a Fairchild 35c scoop, ran over roof bolts lying on mine floor, causing roof bolt to fly up striking employee in left eye.
Employee was using a scaling bar to free up a clevis on the back of a loader when it slipped causing him to fall to the ground fracturing his leg. Employee is currently off work.
Employee was walking from the loader to the pitman. As he walked up a small berm, he heard his knee pop. He was evaluated by a Physician and had surgery on March 26, 2010 thus resulting in lost time.
Employee was in the process of loading holes for a shot when he stepped on the side of a rock and rolled his right ankle.
Injured employee stated a rock 12"" thick x 72"" long X 24"" wide fell, stricking his right leg, causing a contusion.
While the individual was rolling up piece of curtain to scoop the entry, he stepped on a piece of coal and twisted his ankle.
The injured was operating the offside of the roof bolter on 15rt. when a piece of rock fell from the top and struck him above the ankle of his right foot. Causing a contusion and puncture.
Started to climb down from dozer, slipped on push beam and fell to ground hitting his head and his shoulder.
Employee was performing permissiblity on a diesel powered rock duster. Employee posisioned him self in a pinch point. The ending result was employee received a fractured left wrist. This is a lost time accident
EE was moving a cement pallet out of his way when he slipped/tripped over some plastic wrap that was on the mine floor. EE pulled some ligaments in his lower back. He did not go off of work until 10/16/2009.
As EE was dismounting haul truck, he injured his right knee. He continued to work, no restrictions. However, he continued to experience discomfort. After further examinations, employee received further treatments and surgery on December 17 and has returned to full duty on January 11.
Worker was working in the box of a haul truck. Walking on uneven surface across the box and sprained knee.
Employees were pulling a 12inch HDPE pipe(protestive sleeve) over an 8inch HDPE pipe using an excavator. While pulling the smaller pipe ""bound-up"" becoming compressed and suddenly released striking an employee standing near the pipe in the leg. The force of the pipe striking the employee's leg resulted in a leg fracture.
Employee walking from parking lot to locker room for start of his shift. As he tried to step across a stream of flowing water with his left foot he felt a pain in his right knee and was unable to walk without keeping his knee straight.
EE had placed tubing on the shoring planks, he forgot they were there and he turned around and stepped on the tubing and slipped. As he caught himself he over extended his shouder.
Preparing to pump down a sump, the pump crew was draining one of the 6"" hose connections in the hard line out of the sump. The flange end of one of the hard line sections slipped out of the employees hands as he was disconnecting the hose, landing on the metatarsal area of his right foot. X-rays reveiled a broken bone on the outside of his right foot.
A repairman and the Ee had a scoop jacked-up on blocks. The blocks kicked out causing the scoop to drop 3 to 4 feet. Ee was in the drivers compartment. This sudden drop tossed Ee around in the compartment which hurt his back. He has a compression fracture.
Lifting sample bags in desert bit by unknown insect.
The chute feeding the VSI Crusher plugged, which at that time, the equipment was shut down to clean out the rocks. When the miner was removing the rocks out of the plugged chute, another rock shifted and smashed his finger causing the injury, which required approximately 8 sutures. The rocks in the chute were 2""-8"" in size.
Employee was helping install a culvert pipe near the Scales Area when he bent over to pick up a chunk of mud which was about to fall in the ditch. While reaching for the mud chunk, he felt something pull in his back.
Employee felt pain in lower abdomen after lifting a screen panel. Result was strain of the lower abdomen.
While EE was using a hammer to pound a tooth keeper in on a Cat 992 Loader, a piece of metal chipped off and flew under his safety glasses and lodged in his eye requiring surgery to remove.
A piece of draw rock fell between roof straps striking EE on the head and shoulder. The EE was treated and released back to work the next shift. EE continued to work until 12/16/09. EE went off work with fluid on the spine, ligament torn from spine at L-1, compression received from the rock fall.
Employee was punching holes in a 3/8"" plate and pinched his thumb between a stripper bar and plate that he was punching. He fractured his thumb and was taken off of work.
EE reported: Stopped in the battery charge station to change batteries. As he was walking around in the station, tripped over the battery charger cable, alleges that he twisted his right knee.
He was welding inside a gravel car, when exiting the car his foot slipped and he felt pain in the groin area. He continued to work at his regular futies but continued to have pain. He eventually went to a physician and was diagnosed with a hernia. He was removed from work on January 8, 2010 for surgery to repair a right hernia.
Employee was getting off his service truck when he slipped and felt something pop in his left knee.
EE was working on a forklift and the fork on the forklift fell off and hit him on top of his left foot.
Employee was getting off the haul truck and missed a step. The employee fell and hit his head.
Employees's shoulder was strained when a choker he was hooking up was pulled tight.
Employee was driving a 190 ton end dump. He was on the fill dumping his load. he said that the truck bed slamed back and jerking his neck. He was dizzy, vision was blured, and light headed.
Operator tripped on the top step of the ladder while mounting a 992 wheel loader. He fell forward against the cab, causing pain in his right arm.
Employee was pulling on a breaker bar to loosen a plug. When it broke free he felt pain in his upper left arm. Employee was treated and returned to work. He was classified as a lost time as of 10-26-09.
Employee was loading roof bolts into the carousel of the roof bolter. The carousel became hard to move, he jerked on it to make it move and felt a pain in his lower back.
Employee complained of right knee pain. After medical evaluation was done it was found he had a strain due to repeated trauma attributed to his activities as a coal miner. The MRI showed effusion to his right knee.
Supervisor and laborer were unloading T-way roof beams off of a scoop. While pushing beams off scoop, the laborer got his middle finger and ring finger of his right hand caught in between the beam and the light guard on the scoop.
Employee was in the surface warehouse & picked up 2 buckets of roof drill bits. When the employee turned around, he felt a pain in his back but didn't think any thing was wrong. The employee went underground and was helping hang the miner cable. The employee got sick at his stomach and his back started to hurt very bad. Employee brought outside.
Putting hose on suction pump. EE slipped, resulting in hurt/swollen leg.
Carrying Jackleg. Tripped on rock. Initially first aid, went back to doctor when it wasn't getting better and got Positive MRI result for Torn ACLI on 11/4/2009, which made it reportable.
Employee jacked up a three wheel buggy and took the tire off to check for a noise he heard. He had two jacks plus some blocking under the buggy when it killed over and hit him on the left leg in the shin area causing a contusion.
Changing out sieve screen on 144 deslime screen when he stepped on the cross dam panel causing him to twist his left ankle causing a sprain to the left ankle. (This may be a duplicate report).
Employee was cleaning coal in the pit, he got off of his trackhoe to move a pump hose. When he moved the hose it caused a rock to slide off the dig face and roll down and struck him in the lower left leg and foot.
Injured was using a 4' long bar to dislodge build-up material within Stage 4 North lower meal pipe located on level 7 of the pre-heat tower. The material was lodged approximately 3' above the door used in this accident. When the material became dislodged, it pulled the bar in such a manner that injured elbow struck against the door frame.
Installing rib screen in a pre-drilled hole. While pushing in conventional bolt by hand thru screen, rib fell (2"" thick x 6' long) hitting employee in right knee and ankle.
EE was putting scrap iron in loader bucket and felt pull in abdomen.
Employee was helping put steel beams up using Hieuzman Jacks when the jack slipped out causing the steel beam to strike him on the back.
MECHANIC WAS TRYING TO TAKE A TRACK APART AT THE MASTER LINK. THE MACHINE HAD A DEAD BATTERY AND WOULD NOT START. INSTEAD OF MOVING MACHINE UNTIL MASTER LINK WAS IN POSITION RECOMMENDED IN SERVICE MANUAL, THE MECHANIC STOOD ON TOP OF THE TRACK AND REMOVED THE BOLTS. WHEN HE REMOVED THE LAST BOLT THE TRACK CAME APART AND LAUNCHED HIM OFF OF THE MACHINE.
EE was walking underneath some header boards that were stuck to the mine roof from where a brattice had been located and knocked. The header boards fell and struck him on the left arm and back. EE did not seek medical attention until 11/09/09. First day missed was 11/09/09.
Anchor cable was being moved. Employee was pulling cable as it was coming off winch. Employee complained of soreness in his back Monday --- Informed me that he thought it happened while pulling the cable.
Employee was in the process of cleaning out a shelter hole along the track haulage. His left foot was placed on a coal shelf on the mine bottom by the shelter hole. As he reached down to pick up a large piece of coal toward the back of the shelter hole, his left foot slipped causing him to hyper extend his knee. The employee started missing work on 10/10/09 due to this event.
Employee was at the scoop charger on 002 section. After changing the scoop batteries, the employee started to get onto the scoop and a piece of the coal rib fell out, hitting the employee on the shoulder and left lower part of back. Strained muscle in shoulder and back.
Two employees lifting a radiator,weight sifted toward the injured employee landing in his arms and abdomen. Employee felt a pain below his umbilicus. Employee continued to work his shift. Pain became worse over the next week and he reported it to his supervisor on Friday afternoon 10/16/09.
EE was shoveling from the portable plant and slipped and fell striking his chest against the steel frame.
Spider bite while in load out tunnel.
Injured tripped/slipped going through the split in the curtain at the section power center.
Employee was sliding return roller off of shelf and it began to fall, when he caught the roller he felt a pain in his right shoulder. Employee attended physical therapy and then had surgery to repair damage.
She felt like the injury occurred while moving things out of the way to be able to sweep and clean properly in the shop. While doing the cleaning there was only some aching feeling just from sweeping a lot and grabbing and pulling things around. The real pain and swelling did not occur until later in the day when she woke up. Her hand was hurting so she went to the emergency room.
Employee was stepping onto the beam of the highwall miner and ran into the fork of the forklift. The fork struck him in the top of the head causing a laceration that required 4 staples.
EE was lowering a beam from the blend silo shoring system when the rigging he was using slipped. EE grabbed the beam with his right hand causing his right bicep to over-extend.
The injured EE stated that he was shoveling material from underneath the belt conveyor when he slipped and fell landing on a 4"" crib block. The crib block was under the foot of a piece of support structure.
The injured was riding in a man bus that was being used to pull a 400 ft. long steel cable at the mouth of 9 right. the cable got caught on the guide rail of the switch, and the jerking of the man bus caused the injured to come forward and he struck his head and chin against the man bus. The injured has 26 sutures.
IE & another employee were working in a large discharge chute removing rocks to clear a rock jam. Employees were casting 4"" to 11"" basaltic rocks out of their respective sides of the box. The other employee attempted to cast a larger size rock over the side of the box that IE was positioned. The rock did not clear over the side, fell & struck him on his left shoulder.
The employee was up in the face writing some notes down in his book when a shuttle car missed his turn going back to the feedeer and hit the employee that was up in the face. The employee's leg was broke in this accident.
Employee notified supervisor that he had had surgery on his arm, and that the doctor told him it was job related. The doctor said it was nerve damage from laying arm on window seal of truck while driving.
EE was finishing the mechanical repair to a D8 Dozer (tighten fan belt) when his pry bar slipped and he lost his balance. This caused him to fall off the track of the dozer. Upon hitting the ground his left leg broke just below the knee.
EE was grinding a metal stud and got metal particles embedded in left eye. EE went to emergency room on 10/11/09.
Injured was using comealongs to move spill tray, spill tray spun around smashing his finger between other spill tray.
Pumping water out of containment area. Ladder slipped, causing employee to fall on his wrist.
EE was working on a scalper deck of a crushing plant. He got his finger between a bar and hammer and lacerated his right middle finger. EE did not report this injury until 10/13/09 when it became infected.
EE reported: While attempting to move up next row of bolt location,he stepped in a hole twisting his right knee.Contacted the mine following morning reporting he would not be at work due to his knee being sore and was having trouble lifting it up.
Employee was installing 12 x 12 blocks of wood under 4600 shovel. He pushed a block under body of shovel and caught right index finger.
Worker had finished fueling a Cat 14h motor grader, while dismounting the machine using the steps, his right foot slip off the last step resulting in a sprained right ankle.
EE was tramming a scoop tractor when the machine ran over an uneven bottom causing the machine to bounce and the EE struck his hard hat against the roof.
An operator was responding to an alarm on the M3 belt when he slipped on 3/4 inch nut that was left on the diamond plated metal floor. The force of the fall broke the team member's hip.
It was dark (before daylight). Driver drove off edge of road and the truck laid over on it's side.
As he walked around some material citim stepped inot a hole in the grating that workmen were replacing. He fell and struck his chest area as left knee.
As employee was dismounting his haul truck, the catwalk broke loose under his feet. He fell and broke 2 ribs, fractured his tailbone, sprained his hand and hit his forehead. The catwalk came loose during the course of the day.
Walking to job located near elevator bottom slipped on level ground and hurt left knee. Cause was concentration be aware of mine floor conditions. PPE not a factor, Compliance of rules/regs not a factor, mining equipment/system not a factor, miner profiency a factor. Miner worked one day then did return to work scheduled 10/11/2009 afternoon shift.
Employee was sitting in a 375 Komatsu dozer at the beginning of shift. The other equipment was parked in line behind him. A D11 dozer was parked directly behind the fill dozer and in the process of getting out of line and to the work area, his blade hit the ripper shank on the 375 dozer. At 7:49 pm the employee notified the foreman that his neck was hurting and he was taken to ER.
EE was loosing a take up screw on the conveyor chain of a shuttle car when he felt pain in the middle of his stomach. The ee started missing work on 10/15/09 when he had surgery.
Employee rec'd laceration to lower left leg when struck by piece of metal he was working with.
Employee needs shoulder surgery from degenerative changes over time. He alleges that he was working on welding a bucket of quarry endloader when he took a large block of cribbing and moved it around to set under the bucket which caused pain. Surgery scheduled 4/2/2010
Employee was working on utility lines at South Road 7 sump area when loose material fell from back striking employee. Employee was air lifted to the hospital and he was diagnosed with a fractured collar bone, crushed finger and broken rib.
Injured was using a sledge hammer to help take a cutter chain apart. As he struck the pin, the pin went sideways, thus allowing his hand to strike chain.
Employee was washing down floor at the pre-plant when the water hose slipped from his hand and a metal fitting hit employee in the nose and right cheek. Broken Nose
Employee tripped over a phone cord and fell, dislocating her shoulder. Surgery date and days away from work are pending at this time.
Employee was dragging cut belt and tripped over spool of cable and twisted his ankle.
EE ran over a roof bolt with a scoop, it flipped upward striking him in the right side of the face.
Mill maintenance employee was working on a 14' pipe line and a pump that sanded up. Discharge line slipped out of the upper fitting. In trying to lift the pipe up with the overhead crane, the pipe joint came apart. Pipe came down on employee's hand before he could get it out of the way. Causing fractures and lacerations.
On October 6, 2009 the heel of employee's left foot got caught on a step causing pain in left knee. Because of symptoms worsening, he was referred to an orthopedist and was diagnosed with a torn meniscus. He was admitted into the hospital for surgery on November 16, 2009 which is the first day of lost time.
EE was helping unload some cable from a shield car and he tripped over the cable and fell causing a fracture to his left wrist,
Employee was putting two augers together, they weren't aligned properly and one of the augers sprung striking the employee's left thumb.
EE was roof bolting on #3 unit,#2 rt. face. EE pryed draw rock with channel locks and rock fell mashing his left thumb on top of canopy. The EE sustained a simple fracture of the left thumb.
EE was in process of tramming double head roof bolter into the #4 heading. After arrival in heading a rock measuring 1x1x3/4 fell from mine roof, striking ee on left hand, resulting in hairline fracture of left thumb.
The employee picked up a cardboard box and threw it onto the batteries on the scoop. The box did not go all the way upon the equipment. The employee pushed the box with his right hand and felt something pop in his chest.
EE was unhanging bolter cable from the roof when his foot got stuck in fire-clay bottom. As he was pulling his foot free he twisted his left knee. Later during the same shift he again got his foot stuck while he was hanging the bolter cable and again twisted his left knee. EE is not off work as of 10/7/09 pending results from MRI.
Employee was pulling on the miner cable with wet gloves. While pulling on the cable his hands slipped off the cable causing him to lose his balance and fall backwards on his back and straining it. Employee is off awaiting an MRI.
EE stated that while climbing the ladder to get in his truck, he felt pain in his right shoulder.
Employee was positioned on his knees while operating roof drill. He turned his body to the right and felt pain in his right knee. Employee finished his and worked his next shift before missing.
While hanging miner a cable and two ( 2 ) car cables the employee heard a pop and felt a pain in his back
EE was pushing in a shot when he backed into a rock causing a quick jerk to his lower back. EE is off work pending the results of an MRI.
Contractor was waiting to remove stone from discharge table of stone splitter. When the stone rolled on the discharge table after being split, he had his right hand resting on the table and the stone rolled onto his middle finger. When he reacted and pulled his hand away, he pulled the skin off the tip of his finger, requiring sutures to his right middle finger.
night time security guard (an employee of The Ira E. Clark Detective Agency, Inc.) was walking up steps to guardhouse when he slipped and fell causing a laceration to his right arm requiring sutures.
EE was working on the Mehtane Sniffer when a piece of rock fell between roof bolts and struck the employee on the head and upper back.
Was removing the plugs from the gear case when the ratchet slipped striking him in the head and ribs.
While climbing into scoop injury was grasping the hand holds to assist in getting into the scoop. While his hands were in the holds a piece of draw rock fell from the roof and struck his left hand. The appx. size of the draw rock was 1'x2'x2.5"". The rock hit his ring and index finger causing fractures.
EE was putting a roof bolt in a trimed off corner of an intersection. He bent over to pick up the roof bolt after drilling his hole and felt a pain in his left shoulder and neck. He continued to work from the date of the accident until Monday, 10/19/09.
EE was on man trip traveling the intake escape way, man trip bounced over a rock causing ee to hit his head on the mine roof.
On 10/4 EE was being loaded by the shovel when the shovel bucket made contact with the right rear corner of the bed jarring the EE. EE reported incident to supervisor but did not want to be checked out. EE came to work on 10/5 said his back was sore. EE went to clinic. EE was taken off work.
Employee moved cable that he thought was bolter cable. When the bolter was turned on, the cable jerked and threw the employee to the ground. When he fell he landed on his left wrist resulting in a fracture.
Employee was positioning a jack under the left front suspension on the haul truck when he pulled down and back on the jack to roll it under the truck. He felt a pain in his lower right side.
EE was helping with belt move and was assisting with lifting part of the belt structure when he felt a pull in his lower back. Physician took him off work effective 10/05/2009.
Member was working in the #4 entry in the intake near the elevator when he tripped and grabbed top brow as he fell, a rock measuring 32"" X 21"" X 8"" thick fell and struck him in the lower back / pelvic area.
Had hook up crane to engine and was getting down, stepped off torque and stretched lower left leg. It was burning. Worked next day, was going up and down loader and leg started swelling up.
While employee was lifting 50 Lb. bags of ANFO and filling ANFO pot on explosives trailer, felt pain in lower back.
Employee operating motor on track. Motor derailed causing employee to strike head on motor. He had received pain in neck and back. Per 2/5 doc 220093060018, after further investigation, it is believed that base of motor hit rib when motor derailed. The force caused the pole to rotate, striking EE in the neck/back area.
Two employees were using com-a-longs to drag a set of stairs across the top of an overcast, for installation. In the moving process the steps hung up on a lip. IE pushed on the tensioned chain with his foot to free the stairs, as he did the stairs swung to one side. In the process a loose chain (suspended from the stairs) caught his foot and pulled him off the overcast.
The injured employee was operating a motorgrader along a repair/parking lot and stopped and manually moved an old metal ladder weighing approximately 80 lbs. He stated he felt a ""tingling sensation"" in his right arm.
Miner was walking to his loader, when he stepped on a rock, twisting his knee and falling to the ground.
He was tramming the bolter while putting up key hole plates when a rook fell striking him on the right shoulder.
EE started to walk along side of the roof bolter while the machine was being trammed and the EE got his leg caught between the rear of the machine and the rib.
Employee was installing timbers along the # 4 belt conveyor and a piece of coal fell striking his right hand. Contusion on his hand.
Employee was standing in the shop, when he was hit by a 4 wheel ride. * 1/18/10: EE returned to work on 1/17/10. Worked one shift and part of second. Alleged he could not work without hurting his shoulder.
Prill truck 468 was traveling on Northwest ramp when the operator lost control and rolled the truck on it's side.
Shoveling gob onto belt. Shovel went under timber that was loose. Cap piece came off timber and hit employee/contractor on hard hat, jarring head, neck & jaw.
Scoop car operator was unloading a piece of metal in an x-cut, dropped bucket down to unload a hidden post in the gob, got flipped up and struck EE's right foot. Cause is knowledge of knowing post was buried, PPE not a factor, Compliance of rules/regs not a factor, mining equipment/system not a factor, miner profiency was a factor.
The man was operating a personnel carrier when it struck a bump causing the steering wheel to spin and twist man's wrist. The man worked regular duty until 10-16-09 when he reported off.
Employee was working on the left rear final drive of a 777 haul truck when he felt pain in his back, between the shoulder blades. The employee missed the next two scheduled shifts then returned to work.
While clearing an area by Tower 23, employee was piling debris off to the side to make room for the crane when he threw a metal box tubing, which was part of an old walkway cage and the jagged edge cut into his right palm, leaving a deep laceration.
The employee was removing a stone from the walkway when he felt a strain in the right side of the groin. The employee was in the process of lifting the stone when his injury occured. The employee did not begin losing time until 10/6/09.
EE was moving miner cable out of his way when his hard hat fell off. While he was getting his hard hat a piece of rock-coal fell hitting him on the head.
EE was working on the longwall face when his foot caught the edge of an anti-skid plate mounted on the support pontoon causing EE to trip and fall. He used his left arm catching his self to break the fall and afterwards he started feeling pain in left shoulder. EE continued to work until 10/6/09 when he went off work for injury and is off work pending MRI results.
Employee strained left shoulder when he lifted a manhole cover. Employee missed his first day of work on November 6, 2009.

No lost time, is working.
Associate was working in crusher, ladder was let go by helper and ladder slid into crusher. Associate fell, hitting his head on rocks and damaging his arm.
While checking his area fell and hurt back. Lumbosacral right contusion, pain. Restricted.
Employee was unloading the dump truck and suddenly the base of the hydraulic dump hoist broke. The dump truck bed came down in full force. Employee suffered body aches with no visible injuries.
Mechanic was sorting steel beams in the maintenance yard and a piece of the steel slid down and caught his right foot. He worked until 11/6/09 when he was notified he had a fractured tarsal bone in his right foot.
Using 8"" clamp to compress brake caliper on light truck 9590 stated he felt a sharp pain on the top of his right hand. He sought treatment on 10/29/09. Also injured shoulder requiring surgery.
Employee was helping lift a toolbox onto another tool box with him on one side and two people on the other side, causing the weight to be on his side. Reportable lost time injury 3/2/10.
EE set a jack pipe into the rib and got his hand caught between the pipe and the rib.
Employee slipped on ice during a snowstorm resulting in a fractured knee and lost time.
Employee was jacking a belt drive up when the handle of jack flew up and struck employee on right eyebrow and forehead.
Employee was crushed under a magnet hopper chute. Chute had been set upright on one end and toppled over after lifting hook was removed. Chute is 10"" wide and 114"" tall.
EE was under the 4 South No. 2 belt, at about 24 X cut, changing bottom rollers. The area was wet and muddy. A roller fell about 24"" and struck the little finger of the right hand, resulting in an ""open fracture"", requiring 5 sutures and a splint.
An electrician was working to disconnect the transformer from a unit substation and was exiting the substation when a steel frame spacer unit slipped from it's position striking the top of his right hand resulting in a fracture injury.
Employee stated that he was helping the mechanic change the cutting edges on a dozer blade. He bent over to pick up an air impact wrench. As he picked it up, he felt a sharp pain in his back going down into his left leg. He did not report the injury until 11/09/2009 when he incurred lost work days.
EE attempted to open a valve on tank 36. He felt an intense pain in the right shoulder. He was evaluated and underwent reduction of the right shoulder which was unsuccessful. He was then admitted to the hospital for observation and discharged on 10/27/09. First day of lost time will be 10/29/09. He continues off work at this time and will be reevaluated on 11/4/09.
Employee was getting into Cat truck. Opened door, went to step in and missed the catwalk and fell about 6 feet. 4 stitches above right eyebrow, soreness to shoulder, arm and ribs.
Employee and co-worker, was carrying a acetylene tank to load onto the welding truck, when co worker let go of his side and the acetylene tank crushed injury's left pinky finger against the mounting bracket on the truck
Employee stated that about a month ago he thought he pulled a muscle in his right shoulder & he did not report it. On 11/3/09 he was back on shift at work, and he re-aggravated it while operating a jack leg & went to see our medic on site. We sent him to doctor, and work restrictions were applied by doctor.

EE has been on permanent restrictions for several years due to historical knee/joint problems. In December, he saw an orthopedist that confirmed his permanent restrictions and placed him on additional restrictions of no overhead work after an MRI showed a right rotator cuff tear. He is scheduled for right shoulder rotator cuff repair on January 8, 2010.
Miner was assisting mechanic with routine maintenance of rotating cutting edge on a John Deere 824 loader bucket. While the mechanic and miner were rotating the cutting edge it slipped out of the mechanics hands and the miner was unable to maintain his grip on the other end dropping end of cutting edge on his foot, fracturing his right #1 toe.
Employee states that he tripped over a pallet and fell on his left knee and shoulder. Lost time reportable injury Feb 11 2010 due to surgery.

As he put loader in reverse, a clump of material came off the top of the bucket and shattered the windshield. Loader tipped slightly backward and sand in bucket came into cab, crushing console and steering wheel. Employee sustained cut on right arm and fracture/bone chip in right ankle.
GSI Geo-synthetics (sub contractor) EE was traveling across jobsite on ATV. EE attempted to cross small ditch, when ATV flipped over backwards. The ATV was loaded with sandbags on the rear rack. This offset the balance of the machine, contributing to the turn over.
EE was installing bolts on fresh turn in at ZRT break, a piece of rock fell from the top and rib bouncing off the canopy. The rock struck EE on the left shoulder and forearm and slid down his back. This resulted in a fracture of the left radius.
Supervisory employee walked a short distance into the kiln (system shutdown and locked out) to determine if temperature had dropped sufficiently to allow brick peeler to remove built up coating. An employee was driving a dump truck with the dump bed elevated causing it to hit the telephone lines that cross from the telephone post to the office. These cables were detached and hit an employee that was walking toward the office, striking him on the leg and pulling him to the ground. The employee received minor scrapes on his head and bruises due to the fall.

Employee was cutting a right x-cut in #8 entry. He was sitting on a bucket for a seat between the 2nd and 3rd row of roof bolts from the face. A rock fell from between coal rib and between 2 roof bolts striking him on the head and back. The rock was approximately 3'4"" long x 3'5"" wide x 2"" thick.
