A backhoe was incorrectly loaded onto a lowboy trailer whereby the boom was positioned high enough to allow contact with a highway underpass bridge.

During access shaft cage pre-shift (for 2nd shift) two locations on balance rope were discovered to be damaged. Decision made to change rope. Mine evacuated. No injuries or other problems.
The AMS operator was alerted to 19 PPM at BEP #3 in the Gil-8 bleeder. Shift Coordinator was notified and investigated. The odor of burning coal was detected and the order was given to evacuate the mine. Efforts to investigate were unsuccessful, MSHA made a decision to evacute the mine. The cause of the elevated CO was never determined due to the fact the area was sealed.
An outby roof fall was found by the Mine Supt. at approximatley 9:15 am in the East Main return, #2 entry, #30 X-cut, spad 20+80. It measured approximatley 65'x20'x5.5' The fall was caused by an undetected slip in the mine roof. The area was mined in May 2007 and bolted with 6' tension rebar roof bolts.
A roof fall occurred on the #1 unit (1st West Panel) in the #9 entry in the intersection of spad number 17+50 and extending outby through 2/3rds of the intersection of spad number 17+00 measuring approximately 70ft. long x 19ft. wide x 6ft. high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts.
Roof fall in the worked out panel of 1st Left off Pug Mains in the #2 Entry at Spad #7399 Roof fall was 60' X 18' X 4'. No injuries or equipment involved.
An unintentional roof fall occurred in the 2nd SE panel, #6 Unit,#3 Entry and inby the last open crosscut. The area of the fall included both a recently mined, unbolted cut and where roof bolts prev. had been installed. The fall stopped prior to reaching the outby section. The fall meas. approx. 70'L. X 20'W. and 12'-15' H.
IGNITION: The Miner Crew was cutting coal and rock in Cross cut from No. 3 Entry toward No. 2 Entry. They were cutting bottom rock on curtain side when an ignition occurred. The flame lasted approximately 2 minutes and was put out with 1 fire extinguisher and water.
A roof fall approx. 60' in length was discovered in the #5 entry of the #1 butt working section approx 600' outby the face area. The fall did not impede travel or Air flow to the working section.
An elevator was stalled between 2 floors of the Step 1 & 2 Fines Crusher for approximately 2 hours.
An unintentional roof fall occurred in the #2 entry between the number 9xc to the number 10xc of the 2nd Main East. The fall was discovered by an examiner on the weekly examination. The fall was approximately 20 foot in length and 7.5 feet high and 17' 4"" wide.
A roof fall was discovered in the #7 entry on #2 section near spad 731. The fall measured approximately 40ft long by 20ft wide by 5ft high. No injuries resulted from this fall.
A fall of roof occurred in the South West Mains Return at 29 cross cut,# 11 entry. The fall was approximately 20 feet long x 20 feet wide x 8 feet thick. The area was supported by 6 foot fully grouted roof bolts.
Water inundation inby survey station # 1010, # 3 heading in face continuous miner. Cut into old works on right side of cut. Unknown mine.
A trash dumpster fire occured between 1:25 - 1:35 p.m. that burned/smoldered for longer than 30 minutes. The fire was controled using the mine water trucks and equipment, the fire department was called in as it continued to smolder. Damage to the dumpster was minimal. The fire was reported to the district and the 800 number
A high pressure hose blew under the front of the operator's cab on the 992C loader and oil blew back on the engine compartment. The oil ignited and started the loader on fire apparently burning out the emergency shut off. The 992C loader was totalled but no one was injured.
A roof fall measuring 25L x 18W x 6H occurred 2 breaks outby survey station 8811 in the 3rd LT Panel off of the SE NO 4 Panel. The area was bolted with 5 TT bolts.
#1 section was idle on the midnite shift-The section was being pre-shifted/ firebossed prior to day shift entering the mine, when the fireboss found the fall. It was located at the last open break # 4 entry intersection spad # 4339 approximatly 4'1/2 high.
Unintentional roof fall that occured in the intersection of 161. The fall measured 16lx18wx8h.
A rock fall was found in the 1st northeast parallel (parallel main return) in the #1 entry at spad 14+70. These rock falls have no effect on ventilation and are approx 3 miles from active working sections. Size 19' x 60' x 6' to 8'.
Roof fall occured in the inactive workings that extended over an active conveyor sytem. Falling material directly over the conveyor sytem was apr. 20' x 15' x 33"". Acting supervisor who discovered the fall was apr. 1 mile away when it occured and discovered it upon checking the belt conveyor system.
A roof fall occured at cross cut 14 on #1 unit Primary travelway, 40 feet long ,18 feet wide, in excess of four feet.
A rock fall was found in the 1st northeast parallel (parallel main return) in the #1 entry at spad 16+10. These rock falls have no effect on ventilation and are approx 3 miles from active working sections. Size 19' x 60' x 6' to 8'.
The #8 elevator was removed from service for a period of time exceeding 30 minutes for scheduled maintenance to replace the traveling cable to the car. No one was on the elevator at the time and the excapeways were re-routed. The car was tested and returned to service @ 5:52 PM.
A fall was discovered by certified airway examiner in old worked out area. Area was cribbed and dangered off. There was no equipment involved and no interruption in ventilation. This is not a required aircourse.
An unintentional roof fall occured in the 1 South area. The fall occured in the belt entry outby 24 cross-cut. The fall measured 35 ft long by 9 ft high by 16ft wide. **** No injuries as a result of this incident.
The elevator door was not properly shut, which made elavator door sensor to read on and then off multiple times. This tripped the breaker on the elavator, which caused it to stop.
An unintentional roof fall occurred on the 5A belt line at station 15+40 entry #4. The fall did not affect ventilation, passage of men or cause personal injury. The fall did fall on the belt line and will be cleaned up and arched with steel arches. The fall measured 20' x 20' x 7' thick. The area was devaloped in June of 2006. It was supported with 6' fully grouted roof bolts.
At approx. 6:05 am on 4/24/2010 I was told that our elevator went down with 1 employee stuck on it. He was able to get off at approx. 8:30am. The problem was a short in a door switch due to a bad trailing cable.
A roof fall was found in the outby return aircourse in the 9 rooms 2 butt area of the mine by the mine examiner. The fall was approximately 820 feet long x 19.5 feet wide x an unknown height.
An unintentional roof fall occurred in the B-1 6 left section. The fall occurred in the #1 entry 40' outby 2TA.No.6377. The fall is 70' in length X 20' wide X 5' thick. Area of the fall was speared with 4' fully grouted resin bond. Roof speared above the knuckle of the bolt. All accessed are numbered off.
Unintentional fall occured in the intersection of #162 xcut. The fall measured 20'lx18'wx8'h
On 4/23/10 at about 12:30 pm, a bump was experienced in the 1st North Mains at the junction to the 1st North Rooms. The event triggered minor floor heave, rib sluffage and ventilation structure damage.
A roof fall occurred approximately 280' outby the working section in the #3 entry. The fall was 1 break inby survey station # 25975. The fall was approximately 25' long x 18' wide x 10' thick.
On April 22, 2010, the mine experienced an accident when water came from a overlaying mine (#2gas seam) and began flooding the magnum mains inby the mouth of section. The mine experienced an inundation of water causing all mining on the round bottom number 1 section and the miners to be withdrawn. Federal and Stated agency were notified immediately
Trespasser accidentally started the fire, subsequently burned vacant garage at mine site.
A roof fall occurred in the #5 entry at XC26 for 40' X 20' X 8' high, then continued thru the #1 entry measuring 260' X 20' X 8' high, then continued inby thru XC29 measuring 240' X 20' X 8' high, also inclued the 3rd cut thru into the 7th panel and XC28 to the edge of #2 entry measuring 130' X 20' X 8' high.
The trailing cable on the on the #1 man and material cage became caught on a curtain wall panel and was pulled in two. This resulted in loss of power, communication and conveyance of the hoist. One employee was on the cage returning to the shaft collar when the incident occurred. The hoist was returned to the collar within 45minutes.
A roof fall occurred in the #1 entry of the 6th sub panel north at XC20. The fall measured 180' X 20' X 8' high and continued into the XC thru the #3 entry measuring 160' X 20' X 8' high.

The pre-shift examiner found a roof fall in old works, on break 26 on old belt. The fall measured 20'W x 20'L x 6'H. No ventilation, travelways or people were affected by this fall. It was dangered off. It was in a off entry not traveled.
A roof fall occurred in the #5 entry at XC47 in the 6th sub panel north then continued half way into #3 entry for 80'X20'X8' high then continued inby #4 entry for 40'X20'X8' high.
A roof fall occurred in the #2 entry at XC32 in the 6th sub panel north then continued thru XC35 for 210'X20'X8' high then thru #1 entry for 60'X20'X8' high then to the outby edge of XC36 for 30'X20'X8' high.
An ignition occurred at 65+74 on the F-19 Longwall Face at #92 shield. The ignition was instantaneous with no timeable duration. It was self extinguished by the shearer water sprays. There were no injuries to personnel or damage to any equipment. There was a visible sandstone channel ranging from 1"" to 6"" from 90 to 135 shield.
A roof fall occurred in the #9 entry of the 7th sub panel north at XC18. The fall measured 60' X 20' X 8' high and continued into the XC 10' X 20' X 8' high.

A roof fall occurred in the #5 entry at XC50 in the 6th sub panel north then continued thru XC51 for 80'X20'X8' high and included a 20'X20'X8' high part of XC51 between #5 and #6 entries then continued thru #1 entry fo 250'X20'X8' high.
7-10 hoist was down for repairs approximately 7.5 hours. The repairs were made on the hoist.
A roof fall occurred in the #2 entry at XC44 in the 5th sub panel north and continues thru the #1 entry then inby to the outby edge of XC27 for a total of 120'X20'X8' high.

Backed up to get out of way of loader loading dump truck. Visibility was a blind spot and backed into utility truck pushing it 40 feet. Didn't realize that I had run into truck.
2B Longwall had a fall above the anchorage point located in the tailgate entry of the working section at spad number 59+24 to 58+40.
A roof fall occurred in the Main West 1, at 47 crosscut, between # 4 & # 5 entries. The fall measures approximately 66' long, 18' wide, and 7.5' thick.
Elevator stalled for 31 minutes.
A roof fall occurred appx. 3100' outby the active face at break 15 on #5 belt. The fall was appx. 18'x50'x8'. The roof in this area is mixed with no coal rider seam present. All entries leading to the fall have been timbered off and will not be rehabilitated.
An unplanned roof fall in the Coalburg mine on the #2 panel 1 brk outby spad #1180 that measured 80' long x 19' wide x 8' high. No injuries were reported. The area was developed on 9/3/09.
A roof fall occurred in the 6th cut thru at XC52 in the 6th sub panel north, at the edge of #1 entry for 40'X20'X8' high.
An unplanned roof fall coourred in the #5 Entry 1 Break Inby spad 2515 near the Glory Hole. Fall measured 80' long x 20' wide x 8-10' thick. No equipment or injuries. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Area was initially supported with 48"" and 60"" resin bolts. No active section at this location.
Leadman inspected work area & discovered west rib of 4445 Ramp had sloughed in. Immediately barricaded access to the area & reported to supervisor. The affected area was approximately 50 ft in length by 4 to 5 ft in depth. Initial inspection found the ground support, coated swellex bolts had corroded. This area did not impede ventilation and/or foot traffic.
Roof fall 20' x 30' x 4' on the 002 section West Mains where the #3 South Mains has been turned off.
A roof fall was discovered at the 1st Northeast Main xc-7 spad #5+10 #8 entry. The was approximately: length- 18ft, width- 18ft and height- 8ft.
Crappapple portal elevator was unoperational with several miners aboard for a short period of time. No injuries reported as a result of this incident.
A ROOF FALL WAS FOUND IN THE UNIT 1 TRAVELWAY AT X-CUT # 7. OUR PLAN IS TO REROUTE THE TRAVELWAY, AND ALL ENTRIES TO THE FALL WILL BE PROPPED AND CRIBBED.
EE was loading trucks in wet sand area noticed smoke, tried to get #7 loader out of mine and made it as far as 90 and #1 portal and loader started on fire. Put out with extinguisher. No injuries, just equipment damage. Mine evacuated, called MSHA, removed loader.
A roof fall occurred in the 4 right crosscut into the #5 entry. The fall was approximately 8' high x 25' long x 18' wide. The fall did not affect workers, ventilation nor equipment. As a continuation the fall also fell into the #5 heading on 4-22-10 appx. 8' high x 18' wide x 10' in length. The area has been cleaned up and resupported.
A roof fall occurred in the #4 right crosscut into the #5 heading. The fall was approximately 25'long x 18'wide x 8' high. The roof fall did not affect workers,ventilation nor equipment. The fall also continued in the #5 heading on 4-22-10 appx. 10' in length x 18, wide x 8' high.
A continuation of the roof fall reported on 3/17/10 occurred. The fall started at previous fall and traveled 100' outby existing fall. The fall was 16 1/2 feet wide and 6' high. The area was bolted with 5/8 X 72"" fall grouted rebar with 6X6 plates and 2X2X12 cap boards. This area cribbed and dangered off.
The roof fall was discovered in the A Mains Section, #9 block, #4 Intersection. The fall was 3100' outby the section tail. The area was bolted with 8' Super Twist bolts and 12' post-tension cable bolts. No one was injured as a result of this fall. No ventilation or equipment was affected.
A roof fall was found Gasswell Mains, located in old 9 mains, in 3 left, in the # 3 heading. The size of the fall was approx. 16'L x 20'W x 6' thick. The area has been timbered and dangered off.
IGNITION: The Continuous Miner was cutting coal and rock in the No. 1 Entry on No. 8 Section when a frictional ignition occurred. When the operator sheared down with Miner Head to the mine floor, the ignition occurred. The ignition lasted approximately 2 minutes and was put out with a fire extinguisher and wash down hose.
A weekly examiner found a roof fall durring his examination. The fall was found in our M4 Mains x-cut #27 in entry #1. It was 18' X 30' X 15' high. It had been supported with 6' fully grouted roof bolts. The fall did not affect ventilation, passage of miners, did not cause injury, and was not in or adjacent to the examiners rout of travel
When the BTI powdertruck was parked at the end of the shift, a small spark was seen by 2 employees under the operator's seat. All employees evacuated the mine and the equipment operators reacted by extinguishing the flame with 2 fire extinguishers. MSHA was called and communicated to us to stay out of mine.
A roof fall was discovered in the 3H panel the #10 enrty xc-46 spad #31+55. Length-25', width-19' and height-6'-8'.
Unintentional fall that occured between the 2 bleeder shafts. the fall measured 13'wx30'lx8'h.
The roof fell across #4, #3 and #2 rooms by MP #5. There is still a travelway around MP #5. The air is traveling in its proper course. The roof fall was found on the weekly return run.
While conducting second mining with 4 MRS, a fall occurred in the belt heading after taking a right hand ""lift"" trapping 2 MRS. The fall was located just inby SS 905 and is approximately 15"" high x 20' long x 30' wide. There were no injuries and no interruption of ventilation with 2 MRS affected.
Unintentional roof fall at spad #551 20' long 20'wide 7'high.
Fire was reported to the control room at 7:02 AM. Employee access to the area was barricaded off. Flames were coming out of the inspection doors, second mezzanine level of the riser. Raw meal feed rate to the kiln was reduced, biosolids machine was started to reduce the flow of natural gas to the preheater. The fire was contained within the riser by 7:48 AM.
The mine roof fell above anchorage in the return travelway just west of northeast seal number 14
Three grass fires each covering 1/2 acre or less were discovered burning along the railroad tracks. They had started from burning material exhausted from the locomotive stacks that had built up from extended periods of engine idling. The fires were extinguished within an hour of discovery. There were no injuries, illnesses, nor loss of property.
An unintentional roof fall occured in the 4C intake at station 7+70 entry #5. The lifeline and reflectors were re-routed around the roof fall and the fall was timbered off. The fall measured 24 x 24 x 7 foot and had been roof bolted with 6 foot fully grouted roof bolts.
Door switch and closer failure. *Federal and State agencies notified in a timely manner.
There was a fall in the 2nd main east, at spad 102+20 between the 5 and 6 entries. The fall is 14 feet wide, 78 feet long and 14 feet high.
A roof fall was discovered at 2:20am in an old panel by an examiner. The fall was located in the # 5 entry of the old 3T panel. The fall was located between cross cuts 64-65. There were no injuries (or witnesses) or disruptions to ventilation or passage of personnel.
A roof fall was discovered in the 4J panel at xc-24 the #6entry spad #16+31. The size of the fall was length-20', width-18' and height 8'-10'.
An ignition occurred at the face in 89 South Mains while ramping an overcast area. The flames were confined to the face.
The train sat idle waiting to come on site. Carbon built up in the engine. As the train began moving forward it threw sparks into the brush.
Roof fall was at x-cut 11 in 1 West Mains in No. 4 entry. Fall was 20'W x 20'L x 5' Thick. Area was bolted with 5' torque tension bolts with A3 resin. Area was mined in 09/09.
A non-injury alledged face ignition occurred on the 16A longwall near the #133 shield while mining towards the headgate.
A roof fall occurred in the 4A Submains, crosscut #9, #3 entry. The fall measures approximately 20' long, 10' wide, and 5' thick. The roof fall covered belt structure on the 4A belt flite.
The mine roof fell above the anchorage at survey spad 26935 on MMU 008-0.
A roof fall was discvoered at 12:46 AM in an old panel by an examiner. The fall was located in the #5 entry of the 2nd Subpanel No. off of the 4th Panel No. - Old #4 Unit. The fall was located between spads 28+76 & 29+23 (47' L X 18' W X 8.5' H). There were no injuries (or witnesses), or disruptions to ventilation or passage of personnel.
A roof fall was discovered in the number 1 entry at survey station 27839, six crosscuts outby the faces of 034-0/036-0 MMU. Estimated size of roof fall is 8 foot high, 20 foot wide and 120 foot long. No injuries were reported as a result of this fall.
As the hoist operator was bringing two supply cars out, they became uncoupled and dropped into the slope bottom.
A roof fall was discovered in the 3H panel spad #29+25, xc-43 the #9 entry. The length of the fall was 25', width 35' and height 6' plus.
Approx. 3:20, a grass wildfire was discovered on mine property. It had started on another property and spread onto mine property. Fire Department extinguished fire. No mine structures or people were endangered by the fire. MSHA inspector was already at the site when the fire was discovered.
A roof fall was discovered at AB #25 Crosscut, #4 Heading, 3200' outby the face. The fall was 16' x 16' x 12'. The area was bolted with 6' super twist bolts and 12' cable bolts. No people were injured. No equipment was damaged. No travelways or ventilation were disrupted.
A fall of ground accident occurred in the HGB Ramp above the 3370 drift
A small brow fell on the outby end of an overcast, at #4 X-cut on old 9A track. It was about 8' long, 12' wide, and 3-4' high. It was cleaned up and re-bolted.
EE was riding conveyance into mine to trouble shoot electrical failure. Cage became lodged approx. 2/3 down shaft. Safety dogs engaged. Slack hoist rope coiled over cage. Unable to retrieve employee in 30 min. Report to MSHA hotline.
A fall of ground accident occurred in muck bay #2 in the OZY Decline
A faulty ammonia sensor alarmed causing an evacuation of our Underground operation and MSHA was notified as a courtesy. MSHA cited the Meikle Mine for not notifying them about a possibility of an unplanned gas inundation near the Betze Portal within 15 minutes.
A brush fire on the company property was discovered on an inactive portion of the site in the middle of a marsh. It did not effect active mining areas and burned itself out approximately 1.5 hours after being discovered. There was no active work in the area over the last several years.
An examiner found a roof fall in a worked out area of the mine. The fall was in an intersection in our M4-1st Left in entry #8, 2+00. The fall did not cause injury to miners, affect ventilation, and was not in the examiners route of travel. It had been bolted with 5' fully grouted roof bolts.
A roof fall was discovered in the 4k panel between #2 and #3 entry spad #5+51 xc-9. The fall was 25' long,18' wide and 10' high.
Switch lead broken off first floor door cam switch at 3:15PM. No one was trapped. Had no effect on the ventilation of the mine. The elevator was repaired at 4:30PM and back in operation.
A roof fall was discovered in the 1st Southwest Main xc-6 spad #4+20 the #5 entry. The fall was 20' long, 20' wide and 8' plus high.
The #8 elevator was taken out of service at 2:55 p.m. A bad wire in the traveling cable that connects the controls on the car to the controls in the penthouse was found. The wire was replaced with a new conductor and the car was placed back into service at 3:40 p.m. No individuals were on the elevator at the time. All neccessary government agencies were notified according to law.
The injured miner was working at the face area when an isolated portion of the mine roof fell unexpectedly. The accident occurred in an area where roof bolts previously had been installed inby the last open crosscut. The fall measured approx. 70' in length, 20' in width and 12'-15'in height. Injured was tramming continuous miner from #6 Entry to #8 Right Cross Cut. While miner was moving, injured was fatally injured when caught between conveyor boom of miner and coal rib.
Employee was struck when a stair stringer, secure in 4 locations, inadvertently released, striking employee in the chest area, thereby forcing him into a Genie manlift located approximately 8-10 feet from the point of contact with the stair stringer.
Victim died as a result of an explosion, cause unkown at this time. Received notification on 4/5/2010 that our employee was missing. Received notification late 4/6/2010 that employee was deceased.
EE climbed up the side of a conveyor to grease. He then used a crescent wrench to clean clay off a roller. His arm was sucked in. His left arm was removed from just above the elbow. He was airlifted form the scene and was taken to HCMC where they removed the arm up to the shoulder.
Work glove was caught in a pedestal mounted grinding wheel. The first digit of ee's left thumb was injured.
Employee was cleaning dirt build-up around a head pulley using a metal bar. The bar got caught in the pulley, causing the employees thumb to get pinched by pulley head.
I tripped and my hand hit down lever on drill rig. The bit then went down and crushed my foot.
Employee was preparing to do maintenance on miner. While placing cribs under the miner cat the miner fell resulting in amputation of multiple fingers on right hand.
Tried to remove piece of plastic from bag sealer while still running. Lost tip of right index finger.
Employee was dismounting the shift change bus at the beginning of the shift when his foot slipped on the bottom step. When he reached out to break his fall he hit his right hand on the step well panel wall breaking his right little finger.
Employee slipped while stepping to another level working on a coal hauler. He finished the shift and did not file a report until May 3, 2010 when he claimed a ruptured disc in his back. Workers Compensation investigated the claim suspecting a non-work related injury. On 6/1/2010 it was determined the injury was a result of the slip and twist of his back on 4/30/2010 at work.
EE injured his right shoulder while shoveling in silo. He didn't seek medical attention and begin losing time until 5 May 2010.
Claim of back pain due to rough conditions and twisting required to operate haul truck. No report from EE either verbally or on time card prior to 5/3/10. After WSI investigation was determined pre existing condition and employee termination due to giving false information.

Miner was washing out plugged feed tube from sand tank to sand screw from discharge end with water hose. When material broke free, hose whipped out of pipe and struck miner in face resulting in five stitches to left eye.

Carrying a box to the control room and right foot caught the flight of stairs that leads from another floor. Fell on left arm causing rib injuries.
Employee stepped on a softball sized rock and rolled his left ankle. He was looking up while trying to locate a drill hole in the back.

EE was moving an electric motor across the prep plant floor on a flat cart. The EE was pulling the cart by the handle while 2 co-workers pushed the cart. The handle came off of the cart causing the EE to fall against the plant wall striking a wall mounted fire extinguisher.
Employee was exiting a battery car. His right foot got stuck between the operator's seat and the side of the cab as he was stepping out of the car. He lost his balance and his weight shifted to his left leg. He states he heard his left knee pop.

Climbing down from the grade all machine when his left hand slipped from the hand grip causing him to put all his weight on his right arm which in turn caused a tear to the rotator cuff of his right arm. He continued to work at his regular duties until 6/4/2010 when he was removed for surgery to the right shoulder.
EE was going to the warehouse yard in the TS18F forklift yard to load a motor when the right rear tire blew out, resulting in a loud bang. The tire size 1200x20 had a 100 pounds of air. The EE said that he had a headache with some ear pain. EE was taken to the clinic. EE was taken off work for the inner pain.
The employee was airing up a two piece rim for a mack truck. The rim broke and struck the employee in the upper part of his left arm, this broke his arm.

The employee was walking on one of our catwalks and struck his head on an overhead pipe. Employee was properly wearing a hard hat at the time of the incident. He continued to work the remaining shift time. The day after the incident the employee complained of a headache and neck pain but remained at work.

The treating Dr. was not able to determine whether the injury was related to work activity or not and the company will continue to investigate the applicability.
Employee stated that when the roof bolter operator took his cable up, it struck him, knocking him down.
The man was tramming a shuttle car through a crosscut and had the shuttle car get too close to the coal rib. While attempting to tram the car away from the rib, the man caught his arm between the rib and shuttle car.

Employee was injured on 11-03-09 and worked until he had surjury on 4/26/10. Injured struck his right elbow on a piece of metal when he was jerking on hose on a service center. Torn Tricep.

This is being submitted to abate citation # 8026865 - during a part 50 audit. The employee stated that his left foot slipped from the foot hold on the rib protector. He felt a popping sensation in his back. He sustained a back strain and received physical therapy.
Employee stated that when walking to 8 South section, he was walking down the wire side where there was a rail laying. He stepped on the rail causing hin to trip bending his right foot in an awkward fashion. This resulted in fractures to 3 metatarsal bones in his right foot.

Employee was cleaning up debris near the radius switch, when a rock slid off of another rock, striking his right ankle. Note: Employee did not begin missing work until 4-30-2010 and was not placed off work until 5-5-2010.
Employee was bolting off the miner in 4 West. As he was pushing the roof bolt into the hole, the bolt coupler got stuck on the hole in the channel. The employee reached over to push the bolt free from the channel and it sprung catching his left hand between the bolt and the plank jack.
The injured was installing a curtain board and walked around the front of the bolter and beside the ATRS just as the operator moved the bolter. The ATRS pad struck him in the stomach above his navel pushing him against the rib.

Employee was sliding a 25 lb stone from belt to table, when he felt a pull in his left shoulder and neck. Employee was instructed to be off work for 2 days. Employee was released to full duty 4/24/10. Employee did not report to work on4/26/10.

Getting down from cable tray under E-house (the cable tray was approx 3' from ground). Slipped while getting down and fell the approx 3' and landed on left hip.

Employee stated that while working on 2B 8North longwall, he was attempting to put a zip cart onto the handrail of the longwall to transport rockdust to the tailgate. When putting the cart onto the handrail, he caught the last digit of his left middle finger between the cart and the handrail.
Electrician trainee was carrying a 14 pound box of brushes for a dragline motor from the warehouse to his truck, when he felt his back give out, almost dropping the box. He continued to work and his back pain became worse. At 1:00PM he was in for lunch and his back pain got worse.
The employee was using a band saw to cut a steel ""H"" beam. When the employee approached to move the completed piece it fell on his right foot, resulting in a contusion to the top of the right foot.
The employee was operating a shuttle car. He dumped his load of coal at the feeder. He started to turn around in the operators compartment of the shuttle car, when the car started to travel from the feeder towards the face. The employee got his right foot that was hanging out on the operators deck caught between the rib and machine resulting in fracture to right ankle.
Dragline operator was placing the boarding steps into place to start a repair on dragline. He removed the step from the storage area and dropped the steps on his left foot resulting in fracturing two bones in his left foot.

On April 17, 2010 EE was rock dusting the face of No. 1 Entry by hand and struck the backside of his left knee against the battery end of the scoop. EE felt pain, however completed his shift and went to Summersville Memorial to have it checked. X-Rays revealed a sliver of metal approx 1.25"" long and it was removed on 4/20/10. Stiches were required on incisions made to remove metal.
After further investigation it has been determined that a reportable accident occurred to this employee. He was loading a flinger type rock duster with dust on the end of a supply car. When raising the rock dust bags up (40 lbs.) he felt a pulling sensation in upper abdomen. He missed work from June 23, 2010 until July 11, 2010.
EE states that while rock dusting with a 2"" hose, he turned and felt a sting in his lower back.
While shoveling slope belt employee lost his footing and twisted his left knee. He continued to work until 07/27/2010, when he had surgery to repair a torn meniscus and a tear behind the knee.

He was putting a steel insert behind the shaft of the head pulley at the C.R.S. Two employees working on opposite side of solid guarded pulley had hoisted up the pulley when the hoist shifted and broke. The head pulley shot back (due to the conveyor belt) and the plate hit his hands.
Original incident was reported to company on 4/16/2010 but not MSHA reportable/lost-time until 9/14/2010, thus the submission of the report. Incident occurred when climbing down from ladder when his foot/hand slipped off of handrail. Employee caught himself on safety chain on separate piece of equipment parked next to loader.
Employee was pushing in a shot when the dozer he was operating went over a rock. Employee stated that he was jarred.

Employee said his left knee had been hurting for the last two weeks. He stated that he didn't know what caused it but that he thought he hurt it working on an overcast and that he was on his knees a lot. Employee started missing 05-05-10.
EE had been removing concrete blocks from a brattice in order to create a regulator opening. He was using a sledge hammer to remove blocks. He removed his hard hat to wipe perspiration from head/face. While wiping his face, a piece of roof rock struck him on top of his head. He doesn't know how big it was. Received cut on top of head, needed 4 staples. EE missed 4 days.
EE was lifting cathead on the roof bolter cable up and pulled a muscle in his back.
Employee was holding a ladder for another employee. As they were hanging a piece of mine curtain, a rock approximately 18""x14""x1"" thick fell from the mine roof, striking the employee holding the ladder on the left hand.
Employee was working on a longwall shield when a piece of rock approximately 15""x14""x1"" thick fell between two shields and struck him on the right shoulder.
Employee was taking empty, crushed drums off of a roller conveyor and stacking them onto a pallet. As he lifted a drum, he felt a pop in his back followed by discomfort. He went to chiropractor who performed an adjustment and gave him an all-natural muscle relaxer to ease the discomfort.
While cleaning up around the shop compressor, employee stepped on a rock and twisted his left ankle.

EE stated that he injured his back as he was climbing down the ladder of the 777 as he was exiting the truck at he end of his shift. He stated that he was OK and refused medical treatment. He worked his shift the next day and then did not return to work. He consulted a Dr on April 17th and has not returned at this time.
Employee was opening a discharge gate on a rail car. The bar he was using to open the gate, slipped in the gate mechanism. The employee felt a strain in his lower back as he reacted to regain his balance.

He was taken to a medical facility where an X-ray was taken which found a fracture to his knee cap. He saw an orthopedic surgeon on 4-26-2010 and was taken off work.
Employee was tramming the Miner from the deck and had his arm on the cable arm of the Miner when he hit the rib and twisted his right arm. He continued to work and due to complications started missing work on April 22, 2010.
EE was hanging miner cable and injuried his back. He was taken to hospital and x-rays were negative, told to follow up with a specialist, he has done that and diagnosed with a muscle strain.
EE's were installing boundry post when injured EE jumped off the tailgate of the work truck and twisted his right ankle.
After delivering a drill bit and a box of rags, employee dismounted the drill, slipped and fell on his rear and then rolled on his back. The fall is best described as a hard sit-down.
EE was walking down entry (#1 entry XC11 to 12 northwest mains), slipped and fell on his SCSR. Pulled muscle in lower back, starts physical therapy 4/21/10.
While hanging the miner cable in a cable hanger, the employee was struck on the head, neck and shoulders by a section of head coal which came from between the roof straps.
The EE stated that he was pulling on the cable and the cable stopped he then jerked the cable and he felt pain in his back. He left the job site but returned to work the next day and worked the next 4 days. EE didn't come to work on 4/19/10 and didn't call his foreman. The next day the foreman called him, EE stated that he would be off for a week or two requested by a Doctor.
The injured fractured his right leg and bruised some ribs when he slipped and fell after he stepped on the steel plate attached to the foot shaft on a feeder. The feeder was locked out because it had gobbed out and the crew was cleaning it. When the injured fell he landed on the conveyor chain, on his right side and back.

EE was walking through a set of fly pads and walked into the boom of a miner with his head and then told foreman he was dizzy and wanted mother to pick him up.

Operator was driving from the loading point to dump site when the front right wheel dropped into a low spot on the haul road causing operator to lunge forward and to the right. Seat belt stopped the operator and caused a sharp pain in his back and shoulder. Transported to the hospital for evaluation.
He stepped up on the conveyor frame that was about 4 ft high. The conveyor was not working because the generator was down and locked out. When he stepped up he was looking at his co-worker, then fell backwards. He was dazed and confused afterwords. He did not know for sure if he fell or if he passed out.

Employee was standing watching the Cont. Miner cable as it was being pulled out of the excess pile. The Roofbolter had started to tram to the face, when the injured was struck by the bolter cable that was hung to the roof. The cable struck him on the shoulder, arm, and mid-back. He worked until 04/25/2010 when he was taken off for testing.
Miner twisted ankle after falling while standing on a pallet with a rubber mat on it. Moderate sprain to left ankle. Expect miner to return to full duty w/ in 2 weeks w/ no impairment. Miner not wearing work boots per policy. Poor housekeeping. All miners counseled on PPE and proper housekeeping. Nonconductive surface re-engineered to prevent future occurence.
Employee opened a port on a meal pipe to check for plug up and material flushed out burning the employee on the back of the neck, head, left ear and hand.

Employee was trouble shooting a power control panel using a voltage meter rated for a maximum of 600 volts. He used it to check voltage that was 2,080 volts. An arc flash occurred which resulted in second degree burns from his right elbow to his wrist and third degree burns from his wrist to the tips of his fingers. He also has some second degree facial burns.
EE was operating a rock truck when he was backing up to get loaded he backed into the loader.
EE was ""enangotado"" adjusting motor screws with an industrial cricket at the primary crusher plant, when he stood up, he felt pain in the lower back and his left leg.
EE was doing clean up of trash on the beltline. EE bent over his right foot slipped forward. EE felt bad pain up leg to his buttocks now it has went down his leg to his ankle.
Employee was helping pull a broken conveyor belt together with a pulley and wire rope. As they were pulling, the pulley came loose and struck a post, dislodging the post which struck the employee.
While attempting to exit Rock Drill in high wind conditions, door was suddenly pulled open causing employee to lose balance and land on ground off balance. Suffered strain and pain to lower back and left side.
EE was plastering around a door and when he stacked another block on top of the door, the blockbond splattered up under his safety glasses and got under his safety glasses and into his left eye.

I stepped out of my truck and was walking around to the back of the truck when I stepped on a piece of scrap metal that was laying on the ground. I twisted my right foot and ankle.
Injured employee was operating the shear when a rock fell from between the shields and smashed his fingers on the pan line. He was transported to the ER by EMS. Diagnosed with a fracture and lacerations to left index finger. He was hospitalized for observation and released on 04/14/10. He remains excused from work.
Employee was removing a bearing race from a yoke. Yoke was clamped in a vice & employee was knocking out race with a hammer and chisel. His hammer glanced off the chisel, striking the tip of his middle finger on left hand, smashing the finger. Employee was wearing gloves & safety glasses.
In the process of installing a roof bolt off the miner in 2 South rehab area. When installing a cartridge of resin into the hole, he stepped on the step provided on the rib protector and was putting his other foot on the drill mast and when he shifted his weight he felt a ""pop"" on the back side of his right knee.
Miner was lubing screen. When he stepped away from screen, he stepped on cobble twisting left ankle. Miner reported incident on April 13, 2010, at 7:00 a.m. He left the site on 4/13/10 and went to Comp Clinic to get ankle checked out.
Operator was digging into muck pile, started curling bucket back and felt rear end of loader getting light. Some loose rocks came over the bucket, down the boom into lower part of windows hitting the dash and breaking it loose. it came back into his knees. Operator got up and walked away.

While installing a 12' cable bolt the employee pinched his ring finger on his right hand causing a laceration to the palm side of his finger. He stated he was unaware of how or when, but later stated that he may have pinched his fingers between two pieces of drill steel.

Park brake on shuttle car locked up causing tires to lock up and car began to slide down toward the miner quickly. Operator got the brakes released and attempted to regain control of the machine to steer it into the rib. The slick conditions didn't allow traction, the machine slammed into corner rib, causing the operator to be thrown around inside the cab, which caused the injury.
Two employee were carrying bottom belt roller on their shoulders to 2 North Longwall tailpiece. The other employee started to drop roller. Injured tried to catch roller with arm and felt a pop in his shoulder. Shoulder strain to right shoulder.
A bottom converyor belt return fell from the silo belt. The falling roller struck injured on his head resulting in a slight concussion.
EE states that he hurt his back when he lifted two (2) buckets of miner bits. Employee returned to work on 8/16/10 & worked 8/16-19/2010. He called us on 8/24/2010 telling us he was back off work because of this injury (4-7-10) - No new accident.
EE helping to hang cables in front of feeder. After cables were hung, feeder was started and ram car operator told to dump. As car pulled forward, it hit EE in back as he was watching feeder run knocking him down onto coal pile.
Stacking liners on a pallet. One was sideways, tried to lift it on pallet using prybars. The liner rolled the opposite way causing the bar to release. Britt lost his balance and tried to catch himself injuring his left upper arm

Employee was putting cover on a 3920 flinger duster, When his right hand got caught between the cover and a radiator cap. 3 sutures to right ring finger.
EE was loading tram motor for transport. EE asked for a sling to be thrown to him, the sling was thrown to him but one end caught a control lever on the Bobcat. The EE pulled on the sling which activated the control lever causing the Bobcat to run up on his leg.
Employee claims that as he was getting out of his scoop tractor, he slipped and felt pain in his lower back on the right side. Employee continued to work until April 21.

EE was operating a roof bolting machine. The EE stated that while roof bolting, he attempted to swap the head out, but the EE pulled the wrong lever and the movement of the machine pulled and injured the EE's left shoulder.

The continuous miner was stuck, was placing 4' x 8"" crib block under the tracks of the continuous miner for traction, EE picked up a crib block and pulled his back.
Employee's were relocating 55-gallon drums from the ""grease rack' area to a storage trailer. They were using a dolly to move the drums. A drum must be moved from the dolly into the trailer, which sits 6"" off the ground. When the employees lifted a drum, EE felt something pull in his upper left side area.

EE was rolling blinds in Evap. He picked up his impact wrench and as he did so, and felt a pain in his left shoulder. On May 7, 2010, ee was given the following restrictions: No overhead work, limit lift to 50lbs with left hand.
The employee was using a Bit Puller to remove a bit from a continuous miner. As he struck the puller a piece of the hammer broke off imbedding in his hand.
EE was lifting pipe and aggravated a previous hernia injury on lower left side of stomach. He will assume light duty until medical action is determined or taken by doctors.
Employee states while checking a hot roller the shirt sleeve on his left arm got caught by the roller, which resulted in his left arm being pulled into the roller. He sustained two fractures to his left arm which required surgery. Also his shoulder and face were burned while they were up against the roller until the arm was disengaged.
Employee was on restricted duty (no climbing or lifting over 30 lbs) assisting with some fabricating. When he was left alone, he says that he climbed up on an unsecured ladder and it slipped out from under him. The fall caused 2 fractures to his right arm and 1 to a left rib.
While inserting a split set, employee was struck in the chest with the dolly off the jack-leg driver. He reported to our medic & was sent to doctor for x-rays, which were negative. He worked his next shift, then went on days off. Upon returning to work, he said his chest is still painful. We took him to ER, and a Cat Scan was done, reulting in a small fracture to the breast bone.
After making repairs to the gate on tank #21, co-worker EE went down to the level below to jog the gate while injured EE stayed on the platform to test if the gate was operating correctly.EE stepped over the chute and tripped catching himself with his right hand on the lip of the gate at the same time it was being jogged when the gate closed on his hand.
EE was climbing on the screen to tighten screen bolts, when his foot slipped and he fell to the same level, injuring his left knee.

EE was opening pump valve @ Area A pond 1 when the valve would not open. EE used 2 hands to open the valve when the valve broke, EE fell back on left side of back. EE landed on a polypipe on his left back side. EE could not move off the polypipe.
Wrench slipped off the hex nut. When the wrench slipped, the box end of the wrench made contact with the base of the separator. The employee's hand slipped, from the orignal grip position, down the shaft of the wrench at an angle. This resulted in a hyper extension of the thumb on the Employee's left hand.
Employee and coworker were removing a piece of cut rock from the saw table. Employee had his right hand on the rock and when a coworker moved the cut piece of rock, it slammed into his right hand pinning it between the cut rock and the main rock.
Employee deenergized air and power to the packaging equipment. He was retensioning the chain that runs the destacking tray. As he was taking tension off the chain he saw movement in the corner of his eye. He reacted by pulling his arm out of the machine. When he did so his left elbow scraped across the sharp edge of a metal tray.
Boom slipped on a dewatering pump & caught right finger/hand (surgery 6/17/10).
EE was using a ratchet and pipe to loosen bolt. Ratchet broke and hit his finger between the pipe and ground.
EE was climbing down a ladder on a rail car to get the brake. He missed the bottom step with his right foot and landed hard on the ground. EE tore the meniscus in his left knee. EE was not given restricted duty until 5/19/2010.
Lock out tag out procedure was not followed. Miner got his right hand caught in between conveyor belt and slide rollers.
EE was throwing cribs from one side of the entry to the other side when he lifted and twisted wrong, feeling pain in his lower back and right leg.

While standing at the side of the roof bolting machine, the employee reached for his lunch bucket when a piece of rock fell from the mine roof striking the employee on the lower right leg. The employee suffered a fracture due to this incident.

Employee was climbing a small set of stairs and fell, striking his forehead on the concrete floor resulting in a concussion. It was later determined the fall was due to a medical condition.
Employee on blast crew helping load shot. He had just attached the delivery tube to the auger on bulk truck. Employee stepped backward onto a rock and twisted his ankle.
While putting cutter back on miner, the jack slipped and caught the employee's right index finger between the jack and the CLA.

EE states he was pulling 6X6 angle with a choker and the choker slid, smashing EE's right ankle between grating and angle, causing severe muscle sprain. EE received medical treatment and was taken off work.

Employee was working the gates in the tunnels when the injury occured. He was hitting the gates to loosen the sand and make it come out on to the belt. Was swinging a 2lb hammer that caused the problem in his left shoulder.
Fell backwards out of back of service truck while stepping to bumper.
While attempting to retrieve a tie-off line on a barge, a seperate haul line came free and smashed his left thumb between the line and a permanent structure. This incident resulted in a fractured right thumb along with several stitches.
COMING DOWN OFF CAT 988 LOADER TO GET AN ALLEN WRENCH AND LOST HIS BALANCE AND FELL TO THE SHOP FLOOR. EE CRACKED SOME RIBS AND HURT HIS ELBOW.
A rock was stuck in the jaw crusher. EE entered the crusher to break the rock free. The area couldn't be reached with the breaker arm. As the rock broke free it rolled on another rock & struck his knee. He was able to get out & just complained of a bruised knee. After tool clean up the supervisor asked to see the knee & recommended he go to the hospital to check it out.
Removing sheave on pyrite cell using overhead crane. Too much pressure put on sheave, popped up and hit employee on buttocks. This became a reportable when employee had surgery 7-19-10.
While palletizing flat rock (landscape stone) injured person was swinging a maul & twisted his lower back causing a minor strain on lower back muscle.
EE was shoveling at the 15 W belt pony drive when he twisted and felt pain in the left lower side and pain down the left leg. EE continued to work until 6/7/10 went off work due to this injury.
Top head of drill caught employee's foot while positioned on cross member of derrick resulting in multiple foot injuries.
A rock was dumped onto the belt conveyor from the stageloader conveyor and as the rock landed on the belt, it flipped up and struck the employee on the back of his right hand thus fracturing his right ring finger.
While changing ram batteries and with batteries elevated reached beneath them to clean out coal and battery came down on his right hand causing crushing injuries. Transported by ambulance to Springfield Memorial ER where the index finger of his right hand was removed due to the extent of the injury. He was admitted for observation.
On 5/19/10, the employee reported that he required shoulder surgery for the injury that occurred on 4/5/10. The employee stood on the 8-bay controller to unhang a hose from the roof. The employee fell off of the controller.
The employee picked up a 40 pound bag of rock dust and strained his lower back on 4/5/10. this injury became a lost time reportable on 4/8/10
Rock fell between shield canopies and hit EE's hand resting on the hand rail. Restricted duty injury.
Mine examiner was making an examination & was walking on a muddy surface covered with water. He slipped & as he tried to catch himself he strained his left shoulder. He continued to work his normal job until 6/27/10. He was scheduled for surgery on 6/28/10. (MRI report indicates degenerative cyst).
Employee was trying to locate a hydraulic leak on equipment by holding cardboard in the area of the leak. When the hydraulics were activated, his hand was in line with the hydraulic leak resulting in a cut finger.
Employee was working & got something in his eye. He came up to medics and nothing was seen. He went to doctor on his days off and it had turned into an eye infection.
Injured Worker was repairing #6 Finish Mill Separator stems inside the separator which requires the person to be in an awkward position. He was kneeling on metal which he used felt for cushion. His knee became stiff. He reported on 6-15-10 that his knee was still feeling stiff and wanted to see a doctor.
Employee stated he felt pain and a pop in shoulder when he grabbed the bolt with one hand and attempted to twist it.
Two mechanics were removing the suction pipe off the cyclone feed pump. As they attempted to remove the jammed coupling from the box spool, they both had hold of the coupling when the bottom half gave way and caught employee's hand between the coupling and object below.
Forklift driver pushed conveyor into EE's right leg was caught in between the seat he was sitting at and the conveyor.
Employee was building a brattice from concrete blocks. Before the brattice was completed it toppled over and a block struck his right index finger.
EE slipped on a rock falling backwards and hitting his left shoulder on an anchor pin at the #4 belt drive.
Using a service truck boom to lift an idler, a chain hook slipped allowing the idler to fall striking the employee. This incident was found on a part 50 audit
EE was putting a tire on a shuttle car. The shuttle car was being held up by a scoop. The shuttle car slipped off the scoop bucket, resulting in EE's fingers being caught bewteen the scoop tire and the fender.
EE was pushing on man trip (2-person ride) in 6 entry Blk 24 5G section. On hill put fingers between bar guard and battery case, battery slid back mashing 2 fingers.
Employee was repairing cyclone box. He swung hammer to strike the head of a nail, the hammer struck against a pipe below this area, and in doing so, the hammer bounced up striking his left thumb.
The injured was dismounting the rock truck when his right knee gave out. He fell to the ground, twisting his knee. He was sent to dr for assessment and nothing was foung. He was later sent for further assessment to an Orthopedic specialist due to knee pain. He was given a knee brace on 5/13/10 and RTW on 5/17/10.
