The plant mgr noted smoke coming from the shaft and notified MSHA. The smoke was from a planned detonation of det cord tails - plant mgr was not previously informed. There were no miners underground and as we understand definitions in 50.2 no reason to complete a 7000-1. MSHA did issue a 103j order on 08/30/2014, MSHA ventilation study conducted on 10/06-09/2014.

Overhead crane on the service hoist became inoperative. No employees were in danger. Hoist became operative at 6:36am. Was only inoperative for approximately 36 minutes. Hoist was fully functional but a rail box was located on the hoist and the overhead crane mounted to the hoist would not work. Rail car was immediately removed once overhead crane/hoist was operational.
Roof fall occurred at break #33 on #3 belt #6 entry. Approximate size of fall 22' x 22' x 10'.
A lightning strike damaged the Williamsville sub station knocking power to the mine fan and elevator for primary escape way. All personnel were evacuated to secondary slope escape way. Repairs were made and elevator was operational at 7 PM on 8/29/14.
At 1:12 pm on 8/28/2014 a roof fall above the anchorage was found in the North A intake escapeway at spad #13+90. The fall was approx. 30 ft.long x 18ft.wide and 6.5ft.high. The area had been mined in 2005 and was supported with fully grouted resin roof bolts. There were no injuries or property damage.
A D10T Dozer Caught fire pushing down incline.
We found a fall in Panel 1D # 6 entry 2nd cross cut outby. Measuring 6 feet high by 18 foot wide by 40 foot long. No one was hurt, and they cribbed, and dangered off, and have another entry to travel.
On this date, a roof fall was discovered on a weekly exam. The fall was located at crosscut 19 in #6 and #7 entries on the idle section. The fall measured 125' by 19' and was 6' in thickness. The fall was dangered off. The fall was previously supported with 4' resin bolts.
Roof fall was discovered at 13-18 in the #5 entry. Intersection 20'x20'x10'high. Did not affect ventilation. No one hurt.
A roof fall occurred in the 3rd main east at X-cut 30-31 in the pillar line of the 2nd 54 belt line. The fall measured 25'x19.5'x8' and was caused by a hidden slip in the mine roof. The area was supported by 6' tension rebar roof bolts in the pillar line. The area was mined in March of 2008. There were no injuries, no equipment involved and ventilation was not affected.
A fire occurred on a vehicle operated by Austin Powder Appalachia while off-loading its cargo of ammonium nitrate. Efforts to extinguish the fire began immediately. There were no injuries as a result of the fire. The fire did not involve the cargo and there was no release of cargo.
On 8/26/2014 at approximately 3:20 P.M. a rockburst occurred in the 37-281 stope area which resulted in a roof failure and damage to ribs in proximity to burst. The burst occurred in conjunction with blasting a round in the area. All personnel are required to be out of work areas at blasting time. Burst area is approximately 31 feet long and 10' high.
The continuous miner was mining in the third lift of a left pillar when the roof fell causing the cont. miner to be stuck. The roof fall extended from the front of the miner to the back of the main frame.
An unintentional roof fall has occurred on the #1 Section intake primary escape way outby spad 1708. The roof fall was approximately 20ft x 20ft x 12ft. There were no injuries reported and no equipment involved.
At approximately 1:00 p.m. a roof fall was discovered just inby the mine fan canopy.
Material fell out of the roof creating a roof fall. No equipment or personnel were involved. No relevant events occurred before or after the roof fall. The fall occurred at 11th left #1 entry crosscut 39.
The intake phone would not work. The Examiner traveled the intake to locate the problem and found a roof fall. The fall was in the South mains in number 6 entry at spad 3+20. The fall was 18 feet wide by 30 feet long and 10 feet high.
An unintentional roof fall occurred at the #4 entry #145 cx Alma West Mains, spad #144. The fall was 20 feet long, 18 feet wide, and 12 feet high. A plan was submitted and approved to clean up the fall. No equipment or employees were involved in the fall.
The hoist car was out of service for an extended period of time. No persons were injured as a result of this incident
A roof fall was discovered at 12:15pm at the L-seals in front of #3 seal. Spad (2+80) XC # 8 Entry # 3 in the 4th North West Submains. Approx size Length 15' x width 35' x greater than 6'.

A fall of the roof occurred on the 2nd N.E. Mains x-cut 46 #5 entry in the left side return, measuring 19' width by 60' length by 8'-12' height.
An unintentional roof fall occurred on 1 west mains at approximately 9:42am. Fall occurred @ block 74 from intersection of No.8 to intersection of No.9. Fall measured approximately 80' long by 18' wide by 10' high. Roof Bolter was in No. 8 Face. Plan was submitted and approved to retrieve Roof Bolter.
A roof fall was discovered on this date while examiner was conducting his weekly exams. The fall was located on 4-North Mains, #5 entry between crosscuts #10 and #11. The fall was 20' wide by 30' long and was 8' in thickness. Ventilation or persons was affected. The fall is cribbed off.
Elevator once again went down due to a bad drive. A new drive was ordered and replaced the bad one. Elevator was up and running at 6:35PM. There was no one on the elevator at the time of the incident.
Elevator went down due to a blown fuse. There was no one on the elevator at the time of the incident. Fuse was replaced and the elevator was up and running at 9:25AM.
A unplanned roof fall occurred on the #4 section in #5 entry/intersection 70ft inby spad #400114. The effected area was danger off and timber off, The fall measured 6ft high by 19ft wide and 20ft long. A clean-up plan was submitted and approved on 8/25/14, of the effected area. The fall was cleaned up on 8/28/14.
The elevator was removed from service due to a malfunction of the VFD drive. The elevator was repaired and placed back into operation.
A roof fall was found by an outby examiner in our M4 Mains. The fall did not cause injury to miners, affect ventilation or impede passage of miners. A plan has been submitted to MSHA to clean up the fall.
Elevator went down on ""drive unhealthy"". There was one individual on elevator when it went down. He was removed without incident. Elevator was down for 27 minutes, from 10:28PM till 10:55PM.
The Dry Ridge elevator stopped working with an GMS employee aboard. The Employee was unable to exit the elevator for an extended period of time. No one injured as a result of this incident
An unplanned roof fall occurred along the Cool Hill No. 2 Beltline between 43 and 44 cross-cut. the roof fall is approximately 36 feet in length. The cause of the fall has been determined to be from weathering of the mine roof.
At approximately 2:25pm the Schindler Elevator became inoperable. The approach and landing switches were cleaned, re-set the drive, and the elevator was back in operation.
A roof fall was discovered in an outby area at 16-28 in a crosscut from 2 to 3 entry. 16'wide x 21'long x 10'high. No ventilation affected. No one hurt.
At approximately 9:30pm the Shindler Elevator became inoperable. Switches and sensors were cleaned due to high humidity and condensation. The drive was then re-booted/re-set and the elevator was back in operation.
A series of thunderstorms passed over the three pit areas. The coal runs in the bottom were flooded and activity in these areas was halted. The dirt runs, three draglines, and facilities were not affected.
Damage to a phosphogypsum stack berm resulted in the release of an undetermined amount of gypsum transport water. There were no injuries. The breach was closed at 6:25 PM.
EE was attempting to ride passenger elevator to top of silos. The elevator got stuck between floors. The EE was trapped inside from 3:24 am - 5:33 am when he was released with the help of elevator tech from Thyssen Krupp. The elevator needed brake contacts replaced to correct the issue.
Four Contractor employees were performing a shaft inspection in the access shaft (Koepe) when the cage got stuck 1929 below the surface collar. The contractor employees returned to the surface shortly after. There were no injuries as a result of the stuck conveyance.
A scheduled blast at the quarry sent clay material north into the air damaging two homes. A hole was made in a garage wall in one home & a window broke in a second home. It appears that a clay seam occurred in the rock to be blasted. The clay seam went undetected. The blast blew out the side of the wall because of the presence of the clay sea, sending particles into the air.
Maintenance had been done to the north skip, West Safety Dog Shaft the evening of 8/20/14. Around 8:30am on 8/21 the SW Safety dog from the north skip came off of the skip. Plastic bushing liners on the shaft had migrated out of position and the set screws had come loose on the safety dog. The dog hit a guide mounting plate and damaged the plate and guide before coming off the skip

2 EE's were working in N compartment of #1 shaft on work platform about 20' below load-out station. Affected EE's entrapped at that location when hoistman de-energized hoist for W motor electrical fault. Notifications were made.
The elevator at the Shoal Creek portal stopped just below the surface landing as it was being sent to the bottom. The elevator was not getting a run command from the drive. No one was on the elevator at the time and the elevator was repaired and 2:20 pm.
At approximately 5:22am, the Shindler Elevator became inoperable. The elevator was intermittently stopping in the shaft with multiple fault codes. Numerous connections on the elevator were cleaned and tightened. The elevator was back in operation at 9:20am.
An unintentional roof fall occurred on 2 South 2, Crosscut 16 between Entries 5 and 6. The roof fall measured approximately 40' in length, 18' in width, and 8' in thickness. A 103(k) Order (8444691) was issued August 20, 2014, by MSHA Inspector.
Kiln stopped due to Kiln Feed PLC failure. Calciner pressure became positive with ID fan stoppage. Alternative solid fuels in the injection screw started to smolder. Due to PLC failure, fire protection system was not available to stop smoldering. Smoke observed in preheater from ground level and fire department called. No one was injured as a result of the fire.
Entrapment of a person on the Wana Service Elevator for 45 minutes. The cause of malfunction was a stuck slide bar lock switch. (replaced switch)
A roof fall accident occurred on the N-4 longwall (MMU-034-0) in the tailgate entry at this operation on 8/18/2014 at 1:20 AM. No injuries occurred from this roof fall.
An unintentional roof fall occurred and was discovered by mine personnel. No injuries or equipment damage occurred. Fall area measured 8 feet high x 15 feet wide x 20 feet long. This fall area had previously been roof and rib bolted to plan, with roof screen installed. This fall material consists of ""stack rock"", clay seams, with water present.

Oak Springs hoist was out of service due to an electronic issue with the slope car communicating with the control room.
A roof fall occurred above the anchor point on the main line track outby Dupont at survey station 68+00. The fall was 40ft long by 20ft wide and it varied from 10 to 12 ft high. It didn't involve any compliance issues. No equipment involved. No other 50.20 criteria applies.
A non-injury rock fall occurred on the 001 section outby the feeder in number 16 crosscut in the number three entry in the intersection; 15 X 15 X 8' thick.
At approximately 8:55am, the Shindler Elevator became inoperable. The VFD drive was indicating a low line power fault. A loose connection was found on the drive transformer and was repaired. The elevator was back in operation around 2pm.
Found an unintentional fall approximately 20 breaks inby #1 Switch on Doty Mains.
At approximately 12:05am, the Shindler Elevator became inoperable. The elevator doors would not open properly at the bottom landing. Bolts on the outer door mechanism at the bottom landing were tightened and the elevator was back in operation around 1:15am.
A roof fall was found at 12:50 PM on 8/15/2014 of the Buckshot Mains of the Baker room section. Roof fell above anchorage from survey station 12599 to survey station 12600. This was in the # 6 and 7 entry intake. Stopping line was set over and intake was re-established.
Electrical fault in power transformer used for metering at the TR Sub Station. Started at the transformer and tripped all safeties on main switchgear shutting down power to site. Safety checks were made and determined that it was safe to use generators. Hoisting of men initiated & evacuation complete at 9:37am. Final repairs completed 8/21/14.
A roof fall occurred 7 block zone 3 belt entry measuring approximately 50' long 16' wide and 7' above coal seam.
Longwall downshift crew was moving power. While moving the power center out of crosscut 14, several rock props were inadvertently knocked out. Before crew could reset rock props, the roof took weight and fell. Area that fell was approximately 20 feet wide, 20 feet long, and maximum of 4 foot high. This area during advancement had been bolted with 7 foot rebar, resin grouted bolts.
An arc occurred at the outside transformer substation on the oil circuit breaker that resulted in a small fire and smoke. Fire only burned for a short period of time.

The hoist car became inoperative due to electrical/electronic malfunction. All back-up safety equipment performed as designed. No persons were injured or affected. NO production taking place at the time. All other persons were withdrawn from the mine until repairs could be made. The E-hoist remained fully functional during the period.
An unintentional Roof fall has occurred on the 3A Sub-main at the 24XC in the #7 entry. The fall was discovered by the weekly examiner.

An unintentional roof fall occurred in the 4 West area, #3 to #4 entry x-cut. The roof fall was approx 45' long, 10' high, and 16' wide. There was no ventilation interruption, equipment damaged, or injury as a result of this incident. This fall will not be cleaned up.
A non-injury belt fire occurred on the surface, between the silo and train load out building, a distance of 302 feet. The 84"" belt was consumed by the fire and 43' of belt structure was damaged. The fire was put out by the local fire department. The area was gated and locked. The mine is in non producing status.
The Crabapple slope hoist was not operational for an extended period of time. There was a bad breaker that was replaced and put back into service. This slope hoist is not used as any type of escapeway.
The Hydraulic braking system on the hoist was inoperable.
Cave found on preshift - Fallen material a undected slick & slide seen in roof a small coal rider was also noticed on outby side of cave about 1/2 thick or less. Entry witdth inby cave was 17'6"" wide and outby cave was 18' 6"". The slick & slide would not been seen during morning cycle cave was located in #4 entry of D3A but between last open xcut and next one back.
An unintentional roof fall was found in No 2 Entry Return Aircourse, one break inby spad 4709, on No. 2 West-Submains. Fall measured approx 7' high, 19' wide, 10' long. Fall will not be cleaned up. Approaches dangered and cribbed/timbered. The fall does not interrupt ventilation/block escapeways/no injuries reported. Fall is due to weathering of the mine roof.
Crew was in muck cycle of shaft hoistman raised muck bucket off chairs hoist went down, incident is still under investigation cause is not known.
An unplanned roof fall occurred along the 4B track way between crosscut # 38 & 39. The fall was danger and timber off. The fall was 8 to 10 foot long, 18 foot wide and 3 foot high.
The hoist operator inadvertently bumped a power switch when he hung up the pager phone which disrupted power to the accumulator pump. The computer detected this and locked out the hoist. Since it was on a weekend it took the operator considerable time to make notifications and get someone to come in to troubleshoot the problem. See 50.11 report for more detail.
The slope car was moved off of the track by a high lift. During transport some of the wiring was damaged causing the cars electrics to short circuit and damage the breaker for the car. The car was sitting on the surface and not in use at the time of incident. No one was injured or entrapped as a result of this occurrence.
The hoist was inoperable due to drum brakes not releasing.
1830 Day shift blast crew reported fall of ground in L1N preventing egress after 1231 and 1202 shot ore rounds. GF's investigated & reported to management & MSHA as ground support & access was compromised in a small area of the ramp. 3 miners turned off water, air & discharge. UG electricians cut power on the 1308 cable. Area secured, roped off near 1202 sump and 1260 NZ raise.
An unintentional roof fall was discovered during a weekly airway exam. The fall was located in the SW Sub mains 1, 45 feet inby spad D421 in the number 9 entry. The fall measured 42 feet long, 18 feet wide and 7 feet high.
#2 Hoist lost power to console during test trip. Maintenance trouble shooting- discovered water in south brattice box on hoist. Dried water up and replaced solenoid.
The employee was Fire bossing the mine and found a roof fall in the track entry on the 1st East submain @ spad 12+90. The fall was dangered off, the fall was 24 feet long x 30 feet wide.
A roof fall occurred outby in the west mains #1 along the #9 belt in the #5 entry return air course located at spad # 0593. Fall measured 25W 15H 30L.
Operator was removing material from the Borden shot area. He pushed material off the highwall side of work area and traveled over the highwall coming to a rest on the Sewickley bench below. Emergency rescue personnel extracted the operator from the dozer and took for medical evaluation as a precautionary measure.
A roof fall occurred in the return airway between the 12 East and 13 East sets of seals. The roof fall was a continuations of a pre-existing fall but impeded passage through the airway. the root cause of the fall was determined to be weathering.
A roof fall was discovered at 66 wall in 12 East area on the track entry. The fall started in the crosscut and covered 1/2 of the intersection. 18 feet by 18 feet by 10 feet high. No one hurt or ventilation affected.
Roof fall in #3 entry of 3B butt 20 foot inby SS# 16172. The fall is 50 foot in length in #3 entry and continues 20 foot into 3-2 crosscut and is 20 foot in width.
An unplanned roof fall occurred in the #5 entry at x-cut 17 of the S1M4. Fall measures approximately 17' w x 80' l x 7 1/2' h.
A roof fall occurred at 43xc in the South Mains Return airway. The cause of the fall was weathering. the following were not a factor: compliance with rules and regs., mining equipment or systems, job skills, miner proficiency, training, attitude, protective equipment or devices.
An unintentional roof fall occurred on 2 Unit at 35 crosscut in the 5 to 6 crosscut and the 6 to 7 crosscut and the intersection of No. 6 entry. The fall was cleaned up and supported with permanent supports to retrieve 4 obstructed MRS machines.
Production hoist went to manual. Electricians investigated and performed trouble shooting. Problem was identified as brake caliper was out of adjustment. Caliper was adjusted properly and re-secured. Tests were then performed.
Electrician pulled a fuse on the Tube Mill in the Mill Kettle building. When he pulled the fuse, it tripped the power to the plant rendering the main hoist inoperable. The miners evacuated via the air/escape shaft.
At approx. 10:45pm a roof fall occurred on the No.2 section in the No.1 face measuring 17'W 25'-30'L-5'H. This area had already been cribbed and dangered off due to the current roof conditions.
A unplanned roof fall occurred in the 3rd East Panel, Spad # 2+50, Entry # 4. The fall measured approximately 18'W X 18'L X 8' H.
About 3:15 pm., employee went to a storage building to get some planks. He climbed a stack of loaded pallets to get to the planks that were near the rafters and fell approximately 8 feet to the floor below and was found dead the next morning at approximately 6:02 am.
The employee was working on a Fletcher roof bolter. He had the panel board off trouble shooting inside the electrical components of the machine. He apparently came in contact with electricity. The investigation is not complete at this time.
Miner was apparently adding oil to hoist of truck. Box was loaded with rock and not properly blocked. The weight of the loaded box split block and the box came down unexpectedly crushing miner between the truck frame and dump box. Although no damage was done to equipment, miner was killed.
Craftsman was working to replace a wear plate on a shovel dipper door when the cable supporting the plate caught on the door creating stored energy in the system. The craftsman tried to free the plate with a pry bar and as the cable suddenly released; the craftsman sustained a crushing injury to his middle and ring finger(s), by contact with the wear plate and dipper door.
EE was closing the access door on the bag house when he adjusted his hands causing the right hand to slip and bringing the door down upon his left hand. The middle finger of the left hand was caught between the door and the seal edge severing the tip of his middle finger.
Employee's were engaged in installing a pin in an excavator arm. They inserted the pin partially in the hole but it wouldn't go in all the way. The pin popped out and the excavator arm fell on one of the employee's hand pinching it between the two sticks. Employee's thumb tip to the nail, the index and middle finger were severed.
Employee was helping left side bolter on CM make up bolts when a rock (3'9"" l, 2' w, 10"" thick)struck him in the head, neck, right shoulder and forearm, and left hand causing two fractured fingers, several lacerations to the fingers, and severed the tip of his left thumb. The rock fell out between straps. The roof support was installed according to plan.
Employee went home after his scheduled shift on 8/7/14, lifted daughter and felt sharp pain. Dr visit confirms hernia.
The injured employee was working on the head pulley of 3803 conveyor attempting to install a head pulley scraper, when the diverter gate located below the employee hand actuated. The sudden actuation of the diverter gate caught the employee right hand middle finger between the diverter gate and scraper, crushing and partially amputating finger.
Employee was sizing a metal patch for a fan housing. His finger got sucked into the hole and cut the tip of his left, middle finger at the joint.
A mechanic, working on piece of equipment in the shop. Worker was using parts cleaner to degrease a clutch. Worker then violated policy and procedure by using a hand-held butane torch. There was a flash from combustible material that caused burns to the face and head of the worker. Worker was conscious and talking. He was taken to the hospital by another worker was admitted.
Employee was climbing on the gathering head of miner when he slipped causing his left knee to pop. 7000-1 being done due to Part 50 audit.
While backing spot truck under container trailer, employee backed and bumped trailer too hard, causing pain in neck. Also states pain was aggravated by opening trailer doors on 8/30 and 8/31.
EE was using an 8 x 8 Trash Pump, pumping sand and water from a clarifier. The employee, while standing knee deep in water and sand used his foot to push / adjust or move the 8"" suction hose. Shortly thereafter his right foot got caught in the suction flow and pulled his leg into the pipe. EE received a severe contusion that required medical attention.
Employee stated that he was at the shop to wash his truck. He grabbed the hose and was backing up when he stumbled/fell over a drive line laying on the floor. Fell backwards landing on right shoulder. Did not go to doctor until 9/26/14 due to recurring pain. Tendon pulled in right shoulder. Lost time starting on 10/17/14 due to surgery on shoulder, thus reportable.
Vibration from blasting in an adjacent area caused rock to fall from rib, an approximately 20-30 lb rock struck employee in the back of the neck. Evaluation revealed only deep bruising. Later evaluation revealed fractured elbow
EE knelt against rib in the #3 entry watching the continuous miner cable when a section of rib estimated 3' long, 1' high and 8"" thick pushed against his back. Witness came & pushed the rock off his back. Foreman was notified & victim was offered to be taken out of the mine for medical evaluation. He denied medical attention & continued to work rest of shift.
Employee pulled a pallet into the elevator with the pallet jack. As he entered the landing on the elevator, the landing sat a little lower than the ground causing the wheels on the pallet jack to drop. Employee had the handle in his hands, leaned back, and went to pull/jerk the pallet jack free. When he pulled, he felt his right knee give away.
EE was installing a section of 6 inch water line. He reached across belt structure to pick the water line up and felt pain in his groin and abdomen area.
EE was putting crib on a flat car, crib slipped out from EE's hand. Crib caught Rt ring finger between crib and standard. EE went lost time on 9/2
Employee was approximately 3.5 hours into shift and bent to pick up a water hose while cleaning.
Step off of ladder from the 16H motor grader and rolled left ankle sideways.
Employee was working on a bagging system that had sprung a leak. Employee reach behind the machine to find out where the leak was. A clamp came down on his hand and caught his index and ring fingers in a pinch point. Employee needed stitches to close the wound on his ring finger.
At 3am working on plant shoveling. The Big hopper had metal gate w/2 slots (prevents product coming out of hopper). We shoveled dirt off belt. Belt stuck. Opened metal gate & it flew up, EE grabbed it, putting his hand above conveyor & metal gate that was on conveyor
Employee and a coworker were lifting a water pump motor when he felt pain in his lower back. He began working restricted duty on 9-3-14.
Employee was welding (all day) and got a piece of slag in his right eye. He was wearing safety glasses anytime he was welding. He washed his eye out at home that night but next morning still has something in eye so he went to doctor to have it removed. Missed one day of work.
Workman was standing on the #7 head drive pulling on motor leads when he lost his footing and fell into a H beam on the head drive. He strained his right shoulder and lower back.

Approximately 10 seconds after the detonation, a 10' x 4' x 4"" area fell from the roof striking the EE on his left side. The employee suffered injuries to his face, shoulder, back, and ribs.
The employee alleges that he was working with the impact tool and he help the nuts with the finger and the nut got stuck in his finger.
Technician was loosening bolts to remove a hydraulic cylinder on a loader. In doing so, he pulled his ratchet very hard with his right shoulder resulting in pain and a tear.
IE was using a hand held side grinder with a cut brush. Apparently a piece of wire came off the cut brush and embedded in his upper abdomen. He sought medical attention on 9/2/2014 and a piece of wire was removed from his abdomen.

Employee later told supervisor he previously broke his back in a high school car accident. Employee did not tell us he had any physical limitations when hired.
Employee was welding and thought a piece of dirt or something flew behind his face shield and into his eye. He worked the rest of the shift and did not wish to go to the doctor. He worked the following day and after the shift went to doctor on 8/28/14. Doctor found a small a metal shaving in eye.

08/21/2014 Employee was operating a haul truck. Employee felt lower back pain 08/22/2014. He reported his injury 08/25/2014 and was seen by a physician.
He was assisting one of Carter Roag's employees tighten a chain on a conveyor when the grease hose blew off putting grease in his face. He asked the employee of Carter Roag to throw him a rag as he could not see, he removed his safety glasses to use the rag and the rag that was gave to him was dirty and when he wiped his face and eyes it cut his eyeball.
He was driving shuttle car, he had arm laying on outside of cab, rib rolled off and struck his right arm.
EE was attempting to guide a steel cable thru a pulley as he was pulling on new conveyor belt. His hands got into the pulley before he realized it. It broke his left pinky finger and smashed his right index finger. EE started missing on 08-22-2014
They was advancing the track in the track entry and had raised the track rail up with the forklift and the rail puller slipped, allowing the track rail to drop onto his left foot resulting in a hairline fracture to his foot.
Bent over to pick up a bag of rock dust and had pain in lower back.

He stated that he was unconscious for a few seconds. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training or attitude. No PPE or protective devices on equipment involved.
Employee was in scoop scooping up block. The bottom of the deck hit the rib, and the canopy broke loose striking employee in the head.

The initial diagnosis was a strain to the lower back and they were returned to regular duties on 8/16/2014. No medical treatment was given at this time. The employee was further diagnosed and was removed from the duties on 6/9/2015.

While driving a 6-wheeled dump truck, loaded with dirt to back fill a low spot in order to create a road, the ground gave way and the truck fell over on drivers side off a 6 foot bank.
Weld broke on Fletcher bolter head swing jack striking the chest area. Trained on job task. Wearing proper PPE.
While surveying on the refuse embankment, victim twisted ankle and fell. Victim missed work following day to go to Health Care Clinic. X-ray indicated no breaks or cracks. Dr applied ace bandage and prescribed soaking in ice and ibuprofen specific follow-up visits and no restrictions.

This accident did not involve any aspect of compliance with rules, regulations or policies, nor did it involve any mining equipment nor the mining system. The employee had the necessary PPE.
Working in cab on D9N, and started down out of the cab. Stepped down on track and twisted left knee. Heard it pop, but thought it would be ok. Did not feel need to go to doctor. On 8/15/14 he was examined and scheduled for MRI, but returned to work. He missed work 8/19/14 due to pain but returned on 8/20/14 and worked through 8/26/14. MRI scheduled for 9/2/14.

The employee was welding in the surface shop. When a clamp came loose holding a 1/2 plate, striking his left shoulder. He continued to work without restrictions while going to physical therapy. He began missing work on 3/2/2015.
While digging solid rock with bucket, the back of loader was raised up, during that time bucket slipped, dropping cab back down suddenly, jarring operator.
The employee claims that as he was lifting and twisting cement block over chest high, he felt a pain in his middle - lower back. The accident did not involve any aspect of compliance with rules, regulations or policies, nor any mine equipment nor mining system. The accident did not involve any protective personal equipment.

He was wearing gloves. It didnt involve any compliance issues. No equipment involved. It didnt involve job skills,proficiency, training, or attitude.

An employee was working on wet uneven ground located near the highwall miner pit. As the employee began rolling the fuel hose into the rear section of the 2003 Peterbilt fuel truck he attempted to kick the fuel hose to help guide the hose into the hose reel. This twisted him around causing him to loose his footing and fall to one knee, resulting in an injury to his back.
Employee stated he was called back to work after his shift and he climbed 15 flights of stairs to work on an elevator and he felt pain and popping in his left knee. Employee did not report this incident until 8/14/14.
Employee was burned while assisting in removing coal from a silo due to an unexpected coal spill.

Injured employee was operating a shuttle car from the #4 entry to the feeder in the #2 entry. While tramming off a bridge placed on the mine bottom, the shuttle car struck the mine bottom causing a jarring of the employees lower back.

On 8/20/2014 the employee was taken off work by the specialist.
A large rock unexpectedly fell from mine roof and struck the bucket of front end loader while loader was mucking out a shot. The miner operating the loader was jostled inside the loader cab and received sprain/strain to neck and back. The work area had been previously scaled.
Employee was tramming a 2 yd mucker, when he ran over a small rock & jarred his back. Restrictions were issued.
A large rock unexpectedly fell from the mine roof and struck the cab shield of a rock truck while mucking out a face. The truck driver was jostled inside the cab and received lacerations to head and tongue, a concussion, and fractured sternum. The work area had been previously scaled.
Employee was standing on a pile of rocks and foot slipped. Knee twisted and heard pop.
Loose stone was not removed and/or secured prior to entering the crusher. Employee was stuck and trapped by 2 rocks that fell from an elevated position. Employee felt that the rocks were not going to move prior to the accident.
The employee stated that during a belt move he crossed the belt (locked and tagged out) to get a chain hoist and when he crossed back over he slipped and fell striking his right knee on the water manifold. The employee did not return to work on the 13th. This is being reported as a lost time injury.
While roof bolting, a piece of rock fell out of the top and struck him in the face and eye lid, causing a laceration to both his face and eye lid.
Employee was working on clearing a blockage through a 4"" port on the CKD silo cone. Material flushed into employee's face knocking off his safety glasses. Both eyes came into contact with dust.

He tried to reposition the roll and it rolled off of the pallets and landed against his knees pinning him against the wall. Another EE helped him life the roll off of his knees. EE felt something in his back pull when they were lifting the roll.
Worker was holding open the engine compartment door on jaw crusher to check oil. Felt sharp pinch in lower back when he reached for dip stick.
The injured individual was attempting to descend from a piece of chute work onto a tail roller guard following a repair when he found himself to be slipping. He then proceeded to jump from the tail roller guard as opposed to falling from a distance of 4'7"" to the concrete pad. The impact ultimately resulted in a fracture to the right heel.
Empoyee was handing 2 1/2 gallons of fertilizer to employee on Hydroseeder truck when his right foot slipped on back of pickup truck throwing him off of truck onto ground hitting back of head, neck, and back.

Injured employee sustained a hernia while adjusting the forks on a forklift.
The IE stated he was pulling cable slack from a roof bolter while the bolter was being trammed to entry #6-7. While he was walking toward the machine, he was hanging the cable from the mine roof. The IE employee stated he had stepped on some coal material that had been previously mined causing his body to twist in one direction, while his lower leg twisted in another direction.
Employee slipped and landed on his right knee with all his body weight. Conditions were 2"" to 3"" deep muddy bottom. Employee started losing time beginning on 8/20/2014.

While loader operator was digging into muck pile to load haul trucks, the loader struck hard ground in the form of a high wall toe, jolting the loader and rocking the operator. The operator sustained a pulled muscle in his neck, shoulder, and right arm area.
Lifting motor with chain hoist. The chain became disconnected and pinched left index finger causing laceration and fractures.

While loading a shot at the face the employee felt pain in his back. Although he felt OK the rest of the shift the next day he sought medical care as he states he could barely move.

While cleaning out around grizzly wind blew debris into his left eye. Was wearing eye protection at the time. Got medical treatment.

He was taken to the Cobra Valley Clinic in Kearny to be checked out. The Occupational Dr. asked him to get an MRI and after getting results he was to schedule an appointment with Orthopedic.
Employee was moving the continuous miner cable out of the haul road with the help of shuttle car operator when he said he felt pain in the back of his left leg. Employee continued to work until 9-10-2014 when he said his leg was still bothering him and he sought medical attention at Williamson ARH Hospital where he was diagnosed with a pinched nerve.

Operator was operating the skid steer cleaning up around crushing plant, dug down to deep edge of bucket came in contact with a piece of metal in the ground. Operator was thrown forward in cab, lower chest was struck by safety restraint. At the time of accident operator was fine, 1 1/2 hours later he started feeling nauseous went to hospital for exam.

While hanging pipe, employee slipped & he struck his rib-cage (left side) on edge of bucket causing pain & discomfort. Went to medic on site, then to ER. X-Rays were negative-but restrictions were issued.
Was tramming a drill down hill off drill bench at end of short shift and climbed berm turning the drill onto its side. Operator said he was ok and drove himself home. On 08/04/2014 went to doctor because he was still sore and was given the week off work. Neck strain and is cleared to come back 08/11/2014.

The EE was operating a coal hauler. He stated that he was walking on the unit and felt pain in his left knee on 8/1/14. He continued to work up until Aug 8, 2014. He was taken off work on 8/11/14. He had left knee arthroscopy on 8/20/14. A postoperative report received on 8/25/14 along with the attached information indicated wear and tear and degeneration in his left knee.
Employee claims to have stepped off ladder of wheel loader and twisted his knee. Injury did not become reportable until 01/15/2015, when surgery was performed to repair damaged meniscus.
While roof bolting and having already installed the first two bolts of a row, had drilled the inside hole, inserted the glue and turned to get a bolt when the operator was struck by shale on the back of his left leg. His leg was splinted and he was transported to the surface. Diagnosis revealed fractures to the left Fibula and Tibia that will require surgery to correct.
After returning from lunch; employee walked to trash dumpster; reported stepping on rock near dumpster. Rice Medical was immediately call to check out employee's left foot. Report from doctor that there was fracture. Employee was placed in boot; employee was offered and accepted light duty on 10/28. On 11/12, received doctor's release for regular duty starting 11/17.
Miner was lifting a piece of limestone 4-6"" thick weighing approx 100 lbs with the assistance of another miner. He continued working and later stated he didn't feel right in the groin area.

Roof operator was drilling a hole for a 8' truss bolt with a 7' drill rod & a 3' extension drill rod. After drilling the hole the 7' drill rod hung up in the screen he was drilling through. Operator reached for the extension & the 7' drill rod came out of the roof smashing his left thumb between the 2 drill steel and resulted into a fracture thumb.
Material was located on the step of the Cat 725 haul truck resulting in the employee slipping and spraining his right ankle.
As the employee was exiting the cab of the FB005A Flatbed and stepped backwards instead of down causing him to fall landing on his elbow causing a fracture.
Mechanic was working under normal conditions, he was not under any unusual stress or strain. He was installing a drain plug in a wheel loader, wheel differential, which is on a vertical plane on the face of wheel hub. He was turning a ratchet, an felt a pop in his left bicep muscle and the muscle was at least partially torn.
Injured employee was working with partner installing Classifier flights. She was using a 3/8"" allen wrench to hold bolt head while partner used a 3/4"" impact gun to tighten nut. The nut seized causing the allen wrench to rotate resulting in the employees index finger being pinched between the wrench and the flight shoe. The distal 3/4"" of the right index finger was crushed.
Employee was walking through a curtain and stepped in a hole on the other side of the curtain. Causing him to Strain or Sprain his left knee. Employee became Lost time as of 9/24/2014.
An employee was carrying and spreading two 25 pound bags of floor dry. The employee drove to the shop and was unable to move. The employee was transported to Nye Regional Medical Center for observation. On 09-03-2014, MRI results showed a spinal injury and the employee was taken off of work.
He was holding a piece of metal for Guyan Heavy equipment employee when the crane operator made a sudden move and the plate shifted weight.
Employee was carrying a hand tool up a ladder. At 6' the employee fell and sustained minor cuts, and had some significant bruising.
EE was pulling rock props down the longwall face and lifting them over the cable trays into the panline. As he was placing the rock props in the panline he felt pain in his lower back. EE continued to work until 11/06/2014 when he went off for surgery. His first day missed due to this incident was 11/06/2014.
Employee stood up after kneeling, experienced knee pain (popped). Incident reported 8/25/14. Visit to Dr on 9/2/14, with light duty work.
An employee was carrying a 72# shaft and bearing set from a screw conveyor up a flight of stairs. He felt a burn in his groin and was found to have a hernia.
Employee was walking on precipitator when he fell on his right hand and hurt his right shoulder. Employee had surgery on 12/4/2014. 7000-1 being done due to Part 50 audit.
Pump had shut down due to over-temp. fault. Injured read the temp gauge on the radiator & assumed it was safe to open the radiator cap. The coolant had not been circulating and therefore the temp. gauge was giving a false low reading. When the injured opened the radiator cap, hot coolant sprayed out and burned his arms, hands, chest, and abdomen, face & neck.
Loading Haul Trucks. Vibrations in the loader and bouncing around caused back strain.
The IE was opening a set of equipment doors and felt a pain in his left bicep and forearm.He worked until 1-5-15 and was taken off work for surgery.
On 8-18-14 employee and a co-worker was lifting a portable pump onto a flat-bed truck. As they were lifting employee felt a pain in his right shoulder. Employee was seen by a physician and was scheduled for an MRI. The MRI revealed that he has injured ligaments in his shoulder that are going to require surgery to repair. His first day missed from this injury was 8-22-14.
Back pain. Operating Quarry haul truck over haul road. Operator indicated being jarred but could not identify actual area of occurrence. Haul road in fair shape other than small rutting due to rain and stripping activities, but not severe.
Employee was placing the sharp edge of a loader bucket and it shifted pinching the flesh of his left long finger.
While rolling up & tugging on hose in the heading, employee experienced left shoulder discomfort & pain. He did not report this till end of shift, as he thought he could work it out. At end of shift, he went to medic, then to ER-Restrictions were issued(he is currently working light duty)
Employee injured on 8/18 with no reportabilty. On 9/10, after a follow-up medical visit, the employee was directed not to return to work until further healing takes place. Decoupling a hose from a guar line, shut the ball valve, bled the pressure, opened the pump box drain valve, start decoupling hose, was sprayed with residual condensate left in the line in the abdomen.
Employee was mounting a Caterpillar track hoe in the proper manner when his right knee gave way. His knee became somewhat swollen and sore. He was taken to Palms Medical Clinic for examination. X-rays were negative. Rest and icing were recommended. Doctor referred employee to orthopedic specialist. Employee was examined and released to return to work on restricted basis.
An employee was backing up a loader and didn't see a haul truck that the EE was driving. He backed into the haul truck and the EE injured his back.
After shoveling loose material mill employee reported that his shoulder hurt and that he had aggravated a pre-existing, non work related injury. Employee placed on light duty pending additional testing.

Truck parked by bottle cage behind warehouse in gravel. Planted left leg/knee, swung right leg into pickup truck. Left leg gave out as employee was getting into truck. He started to fall but caught himself on truck floorboard. Felt a pop in his knee and a twist.
Using a shop press. the support block shot out, striking the left thumb of injured person, causing damage to thumb and other fingers.
Employee was removing a section of handrail. As employee cut the upright, the handrail fell. The employee grabbed the handrail and strained his back.
Employee was washing spilled material beside a rock crushed. He was 8 feet away from the crusher. A 5 inch rock was ejected from the crusher, fell 12 feet to the ground and landed on his right foot. The rock landed close to his ankle and broke a bone in his right foot.
He was working on a carrier haulage system when he bent over and his back went out.
Injured was removing blocks from wall and a block from the top run fell and smashed his right thumb between two blocks. Restricted Duty as of 8/18/14
EE was operating the primary crusher and using the rock hammer. He leaned over to look out the window at the rock as he used the hammer. He said his back was sore as a result.
Employee was assisting in removing hot coal from silo and suffered burns to his arms and legs.

A SCREEN DEVELOPED A HOLE IN IT. WE TOOK THE OLD SCREEN OUT THEN WENT AND GOT A NEW SCREEN AND LIFTED IT WITH THE SKID STEER TO EE, WHO WAS ON THE CAT WALK. HE LIFTED THE SCREEN OFF OF THE SKID STEER BUCKET AND BALANCED IT ON THE HANDRAIL. HE MENTIONED THAT SOMETHING POPPED IN HIS ARM. I CALLED AND RECEIVED MORE HELP WITH THE SCREEN AND EE RAN THE LOADER THE REST OF THE DAY
While employee was stepping from a platform to load a truck his right foot slipped off the side of the truck causing his right arm come in contact with a guard rail. This caused a contusion on the elbow area and the employee was suffering from pain.
The employee was in the process of roof bolting a heading. After finishing a row of bolts, he went to the rear of the roof boltering machine to get a piece of hog panel. He bent over to pick up the sheet of hog panel, and felt a pain in his back.
Crusher Operator was dismounting the frame of a Tertiary Crusher when his foot slipped and his other foot hung up on the arm guard of the cone causing his knee to hyper-extend.

The injured was pulling on a piece of top belt structure with his hands to remove it from the walkway it was laying in when he felt pain in his left shoulder. This resulted in a tear to 2 muscles in his shoulder which required surgery to repair.
While removing and replacing bucket tips on a 988K Cat loader miner was attempting to remove the retainer pins with a 3 lbs sledge hammer. Upon striking the pin it fractured a sliver off of the retainer and pierced the miners right knee cap. Upon a visit to the clinic it was discovered that the metal was still in the miners leg. Miner had to have surgery to remove the sliver.
Lab tech, moving 3 gallon buckets (weighing 7-10 lbs) from stacked on a pallet, to a pull cart. Some buckets that were stacked 5-6 high fell over onto him as he was kneeling. Buckets hit the employee in the head (wearing hardhat). Headache resulted. No other injuries. Became a Lost Work Incident on August 15th (Dr. gave 10 Days Off).
Employee was attempting to rearrange tin shed, tin slipped causing laceration to left index and middle finger. Lost time as of 08/11/2014.

The employee was explaining work to other miners with his back towards the water line and miner cable. When the miner cable and water line dislodged from the hanger threw the employee into the ATRS jack on the bolter.
The man was loading belt structure into a scoop bucket when he felt a pain in his lower abdomen.
Employee was moving a box of parts located on a shelf. The employee reported feeling pain in the groin area and lower right abdomen.
Employee was cleaning a chute on a conveyor system. He was in a squatted position for several minutes, when he tried to stand up he pulled a muscle in his lower back.
Employee was removing a rock from the walkway of loaded barge. Employee reached down with his left hand to slide the rock off when another rock that was resting on top of the rock he was trying to move released and came down on his left index finger.
Our driver was offloading lube oil at a remote fuel station for the haul trucks. He stepped into edge of trench used by haul trucks as a chock and twisted his ankle. Transported and treated off site with an air cast and pain meds. Further treatment by follow up doctor resulted in a boot and crutches. Released for light duty. Full return expected in a few weeks.
Employee reported lower back pain while pulling a cable. He was evaluated by a physician and released back to work with restrictions.
The injured was picking up rebar close to the base of a concrete pile when she slipped on a loose piece of concrete and lost her balance and fell, catching herself with her left hand causing a fracture to her left wrist.

Employee was loading pieces of loose granite in bottom of quarry into pan. A piece of rock caught his finger between the rock and pan causing injury to the end of his finger.
Employee entered the wrapper to remove an empty pallet. He failed to lock-out tag- out and entered the conveyor from the top walking on the middle of the belt when he removed pallet the conveyor started and pinned his right leg between the exit conveyor and the gravity rollers. He was taken to ER and fallowed up with company Dr. 08/29/2014. He is on restricted work duty.
Employee stepped off low boy onto crib block when crib block shifted causing him to fall.
Miner was removing castable from a quencher spout and piece of castable blew into his eye.
Employee was closing the gate valves on 2-2 ESP dust screw using a pneumatic air gun, when the air gun caught his left glove causing his wrist to over torque. The glove tore away.
EE were loading a flange & valve into the back of a p/u tk. Emp. stood on tailgate & moved the valve so tailgate would close. Valve was positioned between the side of the truck & flange. As he moved it his finger was pinched between valve & flange resulting in a cut finger. Dr took x-ray, the tip of finger was fractured & 15 sutures required. EE placed on restricted duty.
Employee was using a cordless hand drill in an awkward position and the drill got caught up on a piece of metal snagging the drill and quickly twisting his arm.
EE tripped on piece of angle iron sticking out from a pallet of parts by the old scale house after he returned a shove. He fell face down catching himself on his hands and rolled on the ground injuring his right arm. Wrist hurt a little and he shook it off. After about an hour he had pain and could not pick anything up. Took him for X-rays, small compound fracture around elbow.
A maintenance technician was operating a forklift most of the shift carrying empty oil drums from one part of the plant to another. The road that the forklift traveled was bumpy road with several pot holes. Toward the end of the shift, the technician started feeling pain in his lower back but could not remember a single event to cause the pain.
Employee was moving oil barrels in shop and pulled a muscle in his lower left back.
Employee was pulling on a water hose. It got caught on an I-beam. He pulled harder to get it to break free and it did not. The last pull hurt his right Shoulder
Employee experienced shoulder pain while pulling on a barge cable. He sought medical attention on 8/27/14 and was released to full duty until his next appointment on 9/2/14, when he was placed on restricted duty.
EE was on a scaffold in 24 area and squatted down to move into position to replace the old valve with a spool as he turned to face the pipe he twisted his right knee and felt it pop. Strain-right knee
Employee was hosing out shell of 33 Press when he slipped on some mud and his arm came in contact with the press causing a burn.
EE was walking down steps when his foot slipped causing him to fall onto his right shoulder. Causing contusions to EE's right shoulder and neck area.
Employee was working on longwall face and pressurized hose struck him in the left arm and left eye. Restricted duty.

Mechanic was removing cutting edges on the 988H loader. As he was holding the bolts in place with a gloved hand, the bolt head spun, cutting thru the glove into the palm of his hand.
Changing screen cloth, lost balance and fell into hand rail.
Emp. came to work and put on work boots. During tool box mtg emp. felt a burning/itching to his right foot, 4th toe. He felt swelling later during his work shift and removed his boot. Emp. took IBU for pain and swelling and applied ice. On 8/29/14 emp. taken to company Dr. for increase swelling and drainage. Emp. given an antibiotic and placed on restrictions.
Ee was driving haul truck away from shovel, hit a soft spot in the road and was jostled in the cab. He felt pain and it continued to get worse. He was seen at the ER and diagnosed with a strain and released to full duty. Further follow up resulted in a restricted duty return to work release.
While knocking down a pile of clinker, a large piece fell and hit employees ankle.
Employee twisted to the left and experienced pain in his lower right back. Employee was put on restricted duty from 8/26/14 till 9/2/14 and returned to full duty on 9/3/14.

An employee was returning from a break, as he entered the cab an shut the door he caught his middle finger in the door.
Employee was exiting the southwest door of the kiln firing floor to get some vacuum hose. He slipped on some hardened slick lime (from recent rainfall) just outside of the doorway, and fell on his right wrist.
Employee was helping lift a piece of handrail when he pulled a muscle in his back.
EE was taking a sample off of a belt and the sampler hit the belt and was pulled out of her hand. Injuring her wrist.
At approximately 6:00 pm, the employee was at home after working the day shift, and he felt pain in his left knee. The employee utilized a step-ladder with handrails to transfer materials between the 5th floor where parts were staged, and the 6th floor, where the buckets were being changed.
Employee stepped into a drill hole. This did not become reportable until employee recieved a restricted duty release from the doctor 11/20/14.
Employee was at the 10WLW VFD area at the belt drive utilizing the mine phone. Employee while on the mine phone tripped over a golf cart twisting left knee. Restricted Duty as of 8/25/2014.
Employee stated he was walking the intake of Parallel Mains when a rock fell from the mine roof striking him on his forehead and hand.
EE was welding on a pipe fitting with fitting sitting on saw horses. EE bumped the saw horses with his leg & the fitting started to sup. EE tried to catch it & the fitting crushed his right pinky finger.
While rolling on v-belts for the cooler fan, employee caught his left index finger in between the motor sheave and the belt pinching the end of his finger. The injury was to his left pointer finger.
Employee had finished repairing a leak on a machine, crawled out from under equipment, did not experience pain at that time however 15 minutes later experienced back pain/spasms.
Employee was driving a pin out of ripper shank assembly with sledgehammer; when he struck pin and a sliver of metal struck him in his left leg just below the knee. Employee received stitches.
Injured reportedly was climbing a ladder and grasped a rung awkwardly causing him to ""tweak"" his elbow. Injured reported incident on 8/21, requested medical attention on 9/9, and was given work restrictions on 10/8 when the condition didn't improve.
The employee was changing screens and stepped in an open chute hole, resulting in a sprained left knee. The employee refused medical attention at the time of incident and continued to work through his shift.
Employee was laying down in the deck of a scoop installing a kick panel behind the tram pedals and felt pain in his back. He was taken to Dr. and placed on restricted work duty.
Employee was on ground wielding lifting eyes on Jaw Cheek plates. The plates were stacked on top of each other and as he would finish wielding one he would slide the plate off to the side. During this process one of the plates fell on his right foot smashing his right toe causing a Closed Fracture.
Employee climbed on top of the truck to grab the wheel chocks. He jumped off the rear bumper landing on both feet. He did not feel pain at the time, until 4 days later.

Tightening a clamp using a wrench when the bolt sheared off and smashing finger between wrench & clamp.

On 8/19/14 the employee stated he was climbing the fixed, vertical ladder when he accidentally struck his left hand pinky on the ladder rung above his head. The Employee did not request medical attention. However, due to continued swelling and curling of the finger, he later requested and was taken to the company doctor on 8/26/14.

Employee was hand loading 50 pound bags of rock dust onto the scoop. Mine height at location was approximately 44 inches and he was working on his knees. Employee felt a pinch in his lower back and he continued to work.
While roof bolting and adding a 27"" extension was struck on the left hand by falling shale leaving a fracture to the left little finger and a laceration to the left index finger that required sutures to close. He was treated and returned to work with a ""no use of left hand"" restriction.

Employee opened a one-touch door above the separator to release some pressure. As he turned to walk away, material erupted from the chute creating a cloud of hydrated lime around him. Material got behind safety glasses and into both eyes.
EE was trying to push a heavy rock when the rock rolled back and smashed his hand. Another worker took him to the emergency room where they concluded he had an open fracture on his 5th finger.
Employee was changing the tire of a Caterpillar 777 end dump using a hydraulic operated bead breaker to loosen the inner ring. After the ring was loosened he grabbed the bead breaker to keep it from falling to the ground and inadvertently placed his index right index finger between the slide rails while the hydraulic ram was in motion, resulting in smashing his finger.
Employee slipped and fell hurting his foot.
Employee was stepping off of a conveyor when his knee 'popped'. Employee sustained a strained right knee.
The employee lifted a chute to clear it and returned the chute down to the normal position. When he removed his safety harness he felt pain in his left shoulder. The employee was given restrictions on 9-11-14.
Ee was loading broken bags of cement onto forklift on Monday, August 18. He felt pain in his back that day but thought it would go away. It did not, so he reported the injury to his supervisor on Tuesday the 19th. He sought treatment on Friday, August 22nd and was diagnosed with a back strain and released to restricted duty.
Employee was attempting to remove worn feed end top deck screen from Unit #657 vibrator. While employee was pulling, the screen got stuck and caught on a side piece of angle iron. This caused a strain to employee's lower back area. Employee was observed at local hospital and diagnosed with a lumbar strain.
An excavator operator was performing his pre-shift inspection. As he was mounting the 349E Cat excavator, he lost his footing and fell back approximately 1.5 feet landing awkwardly on his left foot spraining his left ankle.
Pulling out the shaft on the spider wheel. Shaft let lose resulting in shaft swing and pinching employee between out rigger on crane and shaft.
