Roof Fall was discovered around 7:30 am on morning of 07/31/2014. Size of the fall estimated at 20' X 20' X 4'. No one was injured and ventilation was not obstructed. All areas near the Fall have been cribbed off, and dangered off so no one can go in the effective area. The fall occured at Spad # 2485.

A roof fall wall discovered in the #4 entry at #41 xcut in the 5 North submains. It is in the intersection and measures 16'x16'x 10' high. No ventilation was affected and no injuries.
A roof fall occurred on the south 60 beltline due to the deterioration of the mine roof from the passage of time. this fall occurred at 53-54xc. the following were not a factor: compliance with rules and regs. mining equipment or systems, job skills, miner proficiency, training, attitude, protective equipment or devices.
Lightning strike during storm, ignited methane bleeder exhaust pipe at number 3 headframe.
Main slope hoist lost power at 4:53pm and was restored at 8:20pm. Fuse had blown on the transformer providing the electric service to hoist.
An unintentional roof fall was found in the intersection of the No.5 entry 2t crosscut 11. The roof fall is estimated to be 20'w x 15' long x5' high. There were no property damage or injuries.
Service hoist kicked out because of a rope slip fault and could not be reset. Tach driver was replaced.
The alarm system on the slope hoist indicated a brake issue. Hoist was shut down and repairs made. The brake adjustment relay out of adjustment. Hoist put back into operation at 1:42pm.
The #1 elevator was taken out of service at 7:40 am. A bolt was broken on the upper door switch. Schindler was notified. EE repaired the bolt. Schindler arrived, tested the elevator, and returned the elevator back to service at 10:00am.
A portion of a rib fell at 12N 4 Block travelway impeding passage by vehicle. The Mine was evacuated and the area was made safe and reopened for travel. No injuries or safety concerns resulted from this event.
On 7/27/14 2-feet to 2.5 sagged onto the top of the longwall stage loader and prevented the loader from advancing normally. The area of sagging roof was approx. 4-feet wide by 70-feet long. The #3 entry of the B-15 headgate panel had been supported with 8-foot fully grouted resin bolts on 5-foot centers with steel roof mesh and cable truss bolts on 5-foot centers.
The Patterson Creek Portal Elevator was out of service due to a power outage that caused a fault in the system. Seven employees were entrapped due to this event and when the elevator began working all seven employees were accounted for.
We reported a roof fall on the White Oak Loaded side of the mines near the Weir. After further investigation we concluded that the fall was not above the anchorage, it is not in a traveled area, and does not interrupt any ventilation for the mines. This was an extenuation of the rock fall that occurred on 22 June 2014. The rock measured 18' X 20' X 5.
Two employees were going up to the top of the shipping silo when the elevator stopped at aprox 8:05 pm. The elevator earthquake safety switch bar fell onto the emergency shut off causing the elevator to stop. The switch was reset and the gentleman were able to operate the elevator as designed at aprox 8:49 pm. They were in the elevator for aprox 44 min.
ROOF FALL: A roof fall occurred at E-6 slant belt cross cut #3. The roof fall was approx. 40 feet in length, 15 feet in width and 2 to 4 feet above anchorage of the roof bolts.
At approx.9:40A.M. on the #2 section a roof fall occurred in the #5 entry. The fall was approx. 20'W X 60'L X 5'-6'H the area was supported with 5' fully grouted resin bolt and supplemental 10' cable bolts. The area will not be cleaned up the area has been dangered off and cribs have been set.
The hoist man took the skip can up into the scrolls to the upper lilly limit and the hoist would not reset with the back out switch. We re-booted the hoist cpu and the hoist reset and came out of the dump.
A non injury roof fall was discovered at break #10 on the #4 belt in the #5 entry. It is approx. 30 ft long and 19 ft wide. The effected area has been dangered off with the intention to be cleaned up and to be rebolted.
A rib roll occurred in the track entry 1 brk outby spad 63 measuring apx 50'L X 16'H X 6'T. No persons were involved or injured. Production was stopped and men were withdrawn outby the affected area. The alternate escape way was immediately rerouted. All approaches were dangered off.
While conducting an evaluation of roof conditions in an outby area of the mine a geotechnical engineer heard what sounded like a compressed air leak. Upon further investigation he observed a broken nitrogen line and an area adjacent to this broken line where the oxygen level was reduced to 14%. The area was dangered off and the N2 supply was shut off. The pipeline was repaired.
Approx. 10:30A.M. on the #2 section in the intersection of #2 entry a roof fall occurred. The fall was approx. 20'W X 20'L X 5'-6'H the area was supported with 5' fully grouted resin bolts and supplemental 10' cables bolts. The area will be cleaned up to recover a Fletcher bolter that was covered up in the fall. The area has been dangered off and cribs set at all approaches.
Lightning strike caused a grass fire on the far west end of the mine property, no buildings or equipment with in several miles of the fire. Local fire department was on site and they determined it would be best to let fire burn out itself this is why fire lasted as long as it did.
Found an unintentional roof fall in the #3 Entry, break #8, of the #26 Beltline.
A roof fall occurred in an outby area of the mine at survey station number 262. The fall was approximately 40 feet long x 20 feet wide and 6 feet high.
A failure at the Pinson Substation caused an interruption to the main power to the TRJV mine site. A problem with the fire suppression switch on the generators prevented the immediate resumption of hoisting.
A roof fall occurred above the anchor point on the 1 East section. It fell in the face of #1 and extended into the 1-2 crosscut. Survey station 26+54. The fall was approx. 60 ft long by 16 ft wide and 8-10 ft high. It didn't involve any compliance issues. No equipment involved. No other 50.20 criteria applies.
Slip fell out, pulling roof bolts, top mostly shale
Roof Fall occurred in the #1 entry of the 10WLW of the belt entry. The fall location starts outby at 5553' to 5586' and dimension is 12'T x 33'L x 18'W. The roof fall will be cleaned and re-supported according to the MSHA approved plan submitted and approved on 17JUL14.
Rock fall was discovered on 1 West Mains at Brk 82 on 3 Belt. Fall measure approximately 3' thick X 10' long X 20' wide.
On the 1 North Mains in the #4 face of the 002 Section, the continuous miner cut into an uncharted borehole which was an 8"" pipe used as a power borehole to the former Humphrey mine. The pipe was sealed accordingly to plan and the area was made safe. There were no injuries.
Hydrochloric Acid was accidentally released into a trench filled with Soda Ash Liquor. A cloud began to form in the immediate area of the tank of diluted Hydrochloric Acid Fumes. There were no injuries or illnesses, however as a precautionary measure we evacuated the immediate area.
Found an unintentional roof fall in the #2 entry of the #3 left bleeder panel.
An unintentional roof fall occurred in the 4 West area, #1 to #2 entry x-cut. The roof fall was approx 80'long, 10' high, and 16' wide. There was no equipment damaged or injury as a result of this incident. This fall will not be cleaned up.
A roof fall was discovered on this date. The fall was located in the #1 entry at the #2 crosscut. The fall measured 20' by 20' and was 6' in thickness. The areas was previously supported with 4' fully grouted rods. The fall was in a common air course, and was not a travel way or an area where miners work. The fall was dangered and timbered off.
Lighting storm passing thru the area must have struck the separator tank on a gas well location. MSHA, fire department and the production foreman of the well were notified. The production foreman from Memorial Resource Development and the fire department made the decision to allow the fire to burn itself out.
An unintentional roof fall was discovered by employee while doing the weekly airway examination. Date and Time is unknown. The fall was approximately 20' X 20' 10' Thick. The area was bolted with 6' fully grouted resin bolts along with 10' cable bolts. The fall is at Spad #442 in the #5 Entry in the 2nd North West Mains
An unplanned roof fall occurred on the 002-0 section, two breaks inby spad # 73724 starting in the #6 face into the #6 to #7 cross-cut. Dimension on the fall was 40' in length, 5' to 7' in width and 8' in height.
A hydraulic hose fitting leaked causing the hoist to malfunction taking it out of service for more than 30 minutes. No persons were injured or entrapped due to this event.
The frequency drive for the hoist motor failed. Miners were evacuated at 6:53 pm. The frequency drive was replaced, the hoist tested, and miners returned underground at 2:05 a.m.
An unintentional roof fall occurred on 2-Unit in the No. 2 Entry. The unintentional roof fall was approximately 14' wide by 6' high by 10' long. The fall was cleaned up and supported with permanent roof support to retrieve two obstructed MRS machines.
A fall was discovered on a pump line. 6th North West Submains (42+40) XC#58 Between #2 and #3 entry towards brattice. Approx length 20', width 18', height greater than 6'.
As employees were arriving for shift the smell of fire was noticed in the shop area. Employees then investigated to discover smoke on the hill north of the maintenance shop. A grass fire was identified and reported to the shift supervisor who then took action to notify the local fire district to extinguish the fire. No injuries or damage to structure occurred.
On or about 7/14/2014 an unplanned roof fall occurred at cross cut # 59 on the 2nd Southwest Sub-mains at the approach to the #1 seal. The area was bolted with 8' and 10' roof bolts. The fall will be cleaned up. The fall was fractured laminated stack rock. The fall was approximately 18'Wide x 18' Long X 8' High.
Roof fall on belt line 8 breaks outby #4 drive. No injuries. Fell between shifts in intersection and 1/2 through crosscuts.
A roof fall was discovered in the 1st North Main Intake Slope approx 150' inby the opening of the slope. Approx length 20', width 18', height 8'to 10'.
Around 1 p.m. employee entered elevator and traveled about 12 feet up from ground level. Power cable caught and pulled out of control. Outer panels had to be cut so employee could get out. Incorrectly gave MSHA time of 12:15 when i called in to report entrapment. no injuries elevator still being repaired, waiting on report from elevator repair group.
The #1 elevator was stuck at the top landing at 3:50pm. Found a broken retiring cam cable. Replaced the cable, tested the car, and returned the elevator to service at 5:40pm.
Non-injury rock fall; outby the face of number two entry; between 14th and 15th cross-cut on the 001 section.
At approximately 5:05 a.m. the Shindler Elevator became inoperable. The door safety switch on the top landing was readjusted and the elevator was back in service.
EE was working/ cutting on a tank and adjacent piping caught fire. MSHA deemed this an accident. Damage is still being assessed due to restrictions with gaining access to the area.
On 7/10/2014 a roof fall was discovered in a travelway entry between break 28 and 29 of the 6 Right Panel of 3 East bleeder. The fall was approximately 6' thick, 18' wide, and 20' long.
Slope car was down due to an electrical problem.
On 7/10/2014 a roof fall was discovered in a travelway entry between break 27 and 28 of the 6 Right Panel of 3 East bleeder. Fall was approx. 4' thick, 18' wide, and 20' long.
At 7:10 am on 07/29/2014, a roof fall above the anchorage was discovered in the North A G entry intake at survey spad #13+30. There were no injuries or property damage. This fall will be rebolted and cleaned up. This area was mined in 2005 and is part of the 7-day exam. It was supported with 6ft fully grouted bolts and appears to have been caused by normal weathering.
There was a unplanned roof fall at the # 5 2nd west parallel at spad 2+57.
A unscheduled roof fall was discovered at 3:54 AM on the Slope Belt 1st North Mains. spad (1+60) XC # 3 Entry # 4. Approx Length 40' x width 18' x Greater than 6'. This fall was on the belt line entry, that was utilized as our secondary escapeway.
Mechanics had done some repairs on a 735 fuel truck. The truck had a Honda 2"" pump on the back and it was fired up to ensure everything worked properly. The pump was sucking air so it was shut off when it suddenly back fired and became engulfed in flames spreading throughout the truck.
A roof fall was discovered in the South Airways in an area where miners do not normally work or travel. The fall was located in an intersection 1 break inby spad 301.
A roof fall occurred (Cool Hill 2 Beltline, 42-43xc)due to the roof deteriorating with the passage of time. The following were NOT a factor in the accident: compliance with rules and regulations, mining equipment nor mining systems, job skills, miner proficiency, training or attitude, protective items or devices on equipment.
A roof fall occurred in the 1st main west at # 10 neutral entry at spad # 63+22. The fall measured 40'x19'x9' and was caused by a hidden slip in the mine roof. The area was supported by 5' fully grouted bolts and 10' cable bolts in the intersection. The area was mined in April of 2014. There were no injuries, no equipment involved and ventilation was not affected.
A unplanned roof fall occurred on the Main North, entry # 5, spad 36+00, x cut 54. The fall is approximately 70' in length x 18' wide x 8' high.
The outside top landing door GIB came out of the door track as the car approached the top landing the outside door contacted the closing linkage of the car door which resulted in the linkage breaking. Repairs were made and the elevator was returned to service 6:39pm
A ROOF FALL occurred on the 001 Section measuring 20' wide X 110' long X 6' high from the #3 intersection through 4L crosscut; one break inby SS 4364 in #3 intersection and one break inby SS 4363 in #4 intersection.
A computer glitch during new kiln commissioning caused a kiln shutdown. Approx. 1200 degree F. air was forced into a rubber duct due to a drop in positive air pressure. The hot air eventually caused the duct to catch fire from the inside out.
On 7/7/2014 at 7:15 a.m. a roof fall was discovered at break 28 in the track entry. The fall was approximately 65' long and 20' wide.
A fall of roof occurred in the 2nd right panel off the 3rd SE Submain at 36 crosscut, # 7-8 entry. The fall measured approximately 40 feet long x 18 feet wide x approximately 9 foot thick. The area was supported with 5 foot fully grouted roof bolts.
An unplanned roof fall occurred on the #1 unit ( Second West Submain) inby the tail piece, Spad (10+70) Entry #8R to #9 Entry. The fall is approximately 35' in length x 20' wide x 8' high.
An unplanned roof fall occurred on 7/7/2014 in an intersection in Old 8 Zone 12 Block @ Spad #9259.
Elevator stopped running at 12:35AM. It would not go up or down. Valencheck was called at 12:40AM. Schindler elevator was called at 2:05AM. Elevator was reported working at 4:15AM. There was no one on elevator when it stopped running.
During the preshift examination conducted on 7-6-2014 after an idle period a roof fall was discovered on Unit 3 in the #1 Entry intersection 80' inby spad 2294.
An unintentional fall of the roof occurred at the #4 entry #36 CX Cedar Grove main line belt. The fall was 70 ft. long, 18 ft. wide and 5 ft. high. A plan was submitted and approved to clean up the fall.
An employee was driving a Komatsu 830E 240ton production truck when it caught fire. The employee stopped the truck, shut it off and safely evacuated the vehicle. Local fire departments were called, they responded and safely put out the fire. This is now under investigation to determine the cause of the fire. No injuries occurred from this incident.
The hoist control HMI overheated. This was noticed during normal PM inspections. Hoist was downed until cause was identified. Air conditioning turned on in hoist house. HMI came back up. Hoist released to duty after it was properly checked.
A continuation of a roof fall occurred at 14-21 area. The fall measured 80 feet long by 10 foot high and 16 foot wide. It was above the anchorage.
EE FOUND THE FALL AT APPROXIMATELY 10:00 am ON 07/14/14. THE FALL IS ABOUT 80 FEET LONG AND ABOUT 30 FEET INBY SPAD #153
A roof fall in the North Bypass track entry from cross cut 32 to cross cut 33 was found on the morning of 7/4/14 at 6:00 AM.
A roof fall was found at CC 28 in the #2 entry on the north mains. The fall was 24 long by 20 wide by 8 feet high. The roof had a slip above bolt anchorage. The area had been bolted with 5 feet resin bolts and supplemented with 10 foot cable bolts.
Found an unintentional roof fall in the #4 Track Entry 3 breaks inby the #4 Track Switch. Fall was approximately 20' wide by 30' long and 4-5' thick.
Roof fall occurred on the 6-North haulage at 24 block intersection 16' wide, 12' high. No persons injured, ventilation not obstructed, track haulage, secondary escape way obstructed, Fall will be supported by arches and track haulage and escape way restored.
A roof fall has occurred in the 2nd right panel off the 3rd Southeast Submain at XC 7 entry #00. The fall measured approximately 20 feet in length x 20 feet wide x approximately 8 feet thick. The area was supported with 5 foot fully grouted roof bolts.
A unplanned roof fall occurred at X-cut #14 on the new way track. The fall was timber and danger off and will not be cleaned up. The fall was 20 foot by 20 foot and 6 to 7 foot high.
Unplanned roof fall occurred in the 3rd North West Submain. Spad (21+00) XC# 30 Entry #5 and was discovered at 10:21. Approx length 18' x width 18' x greater than 6'. This fall was in the belt entry.
Tuesday afternoon at approximately 3:10pm, the mine lost all power due to a problem with the incoming power from the power company. The mine was evacuated and power was restored at approximately 6:00pm.
An unplanned roof fall occurred in 1 West Mains break 42 (SS#357) intersection. It was the length of the intersection & was 5' into 4-3 and 15' into 4-5.
CEMEX loader opr was moving flyash from mat'l storage bldg to hopper. Raw mat'l del driver, ind. o/o contracted w/ Tri-State Carriers(TSC), entered mat'l storage area & exited his tractor, preparing to unload. Loader reversed to get another load & impacted the tractor, striking the driver. MSHA issued Contr ID B0350 to TSC under protest as it is not a contr on CEMEX mine site.
Employee pulled aluminum boat over three electric conductors in dredge pond. A patch on the bottom of the boat cut into one conductor and electrocuted the employee. The employee died as a result of the accident.
As injured was dropping the drill mast the steel came out of the chuck and sprung back toward his right hand. Injured apparently reacted by jerking his hand out of the way and inadvertently placed his hand on the frame of the drill mast and he dropped the drill clamp on his hand as he lowered the drill mast.
He was working in the packing house and wanted to check the flow of material. He open the bin door and stuck his hand up inside and the rotovalve cut the tip of his left index finger off.
EE was working in the packing house and wanted to check the flow of material. EE opened the bin door and stuck hand inside and into the rotor valve and cut the tip of left index finger off. I am filing this form pertaining to a part 50 audit (citation 9316001).
The roof bolter stood on the head of bolter to insert resin and bolt in a 9 ft high area. He asked another operator to raise wrench up with head to push the bolt into roof. After pushing the bolt up, the assisting operator began lowering the head. The victim's foot which was still on the drill head frame became pinched between the frame and hydraulic fitting of the machine.
worker pulled empty bag from full pallet, bag ripped free unexpectedly and we stumbled backwards and felt a pinch in his lower abdomen.
EE was washing the shear off prior to servicing. the shear had more than normal amount of rock on it than normal so he hooked up a half inch hose to the emulsion and turned it on wide open. The hose slipped out of his hands immediately and hit him repeatedly before pressure could be reenergized. This is not a normal operation procedure
An employee was removing a piece of pull point (LB) from conduit when the pipe wrench slipped causing the employee's right index finger to be smashed between the conduit and the wrench.
On 7/17/2014 employee was terminated for excessive absenteeism. Later in the day on 7/17/14 after termination, company was informed employee is alleging he suffered from a hernia. Hernia or incident was not reported to company prior to contact after termination.
While replacing a lift cylinder on a haul truck, employee released tension on chain jack and the employee's left middle finger was pinched between the frame and cylinder head.
The employee was getting off the ground, putting his hand on the pan of the miner when the miner head was lowered striking the employees hand.
Employee was changing an air conditioner filter in the ceiling. The employee was on a step-ladder when he lost his balance and fell injuring his left shoulder.*Miner was given a permanent partial disability rating of 13% from his treating physician for partial loss of use of his left shoulder. Miner was given separation paperwork on 7/7/15.
Employee was replacing a loose liner with two other employees when it shifted and pinched his fingers between the liner and the assembly.
Employee was pulling and lifting rocks with a hooked tool to remove the jam. Originally diagnosed as a left inguinal strain, returned to normal duty as a truck driver. 7/16/14 follow up doctor's visit indicated aggravation of the injury, employee placed on temporary modified duty with restrictions on driving, lifting and repetitive bending.
While working on conveyor the employee reached for a part and felt a pain in his back.
The man was swinging a hammer to install a bit insert on the continuous miner. While swinging the hammer, the handle broke and the man's hand struck a miner bit.
EE was clearing material buildup from a grizzly chute in advance of the primary crusher when a portion of the material dislodged and rolled to a point where it impacted his left ankle against a structural beam. This resulted in a fracture of the fibula at the ankle.
Employee stated that he was walking through a man door located at cross cut 3 on the Main North. He did not bend over enough and struck his head on the top of the door frame. Employee started missing on 09/14/2014
On the nightshift of 7-31-2014 the Haul truck operator finished fueling his truck and accessed his haul truck ladder and claimed to hurt his back the HT Operator reported the incident 8-4-2014 AM. The HT operator was not released to return to work after Appt. on 8-7-2014.
Injured was passing crib blocks through a mandoor when a crib block struck the left hand causing a fracture to the 3rd finger. Injured did not see a doctor until 8/5/14 when it was determined the EE needed surgery.
Employee was driving the long stick over to tower 5 to clean out around it. He started going down a slight incline. He lost control of the equipment causing it to run into/and under conv #14
The 4-A Raw Coal Feeder washed in on Raw Coal Reclaim belt. Employee was using water hose to wash material. Where Employee was standing, material had built up over his feet, and when he stepped out, he felt pain in his lower back and legs.
While Employee was unloading a rib bolter off of a flat car the drill dug into the bottom causing his shoulder to pop. Employee was treated at Raleigh General Hospital.
Moving a piece of a metal sheet and felt a pop in right shoulder blade area.
At the end of afternoon shift, the injured was showering in the women's bathhouse at Sugar Run. She was barefoot and fell out of the shower, twisting left ankle, striking right knee on floor and hitting head on the bench. Sustained sprained ankle. No problem with training, mining machines, PPE or compliance issues.
Employee was hammering in a bit lug on the shear when a piece of metal got into is little finger.
Employee was filling a container with oil when the air powered pump hose broke and sprayed oil in employee's face and eyes.
Employee was hanging miner cable in the #9 entry at spad #9061. The employee felt a sharp pain in his back. The employee returned to work for his next scheduled shift. After his shift on 7/31/2014 he reported to a doctor where he was taken off of work. The first away from work was 8/1/2014.
Injured was assisting the roof bolter operators. As he cut the bands on a bundle of roof bolts the bolter advanced forward and struck his foot causing an acute fracture in his 2nd metatarsal.
EE was walking under CV21.12 and he struck his head on a beam in an area with low head clearance. He was evaluated by a Physician on 7-30-14 and taken off of work. He returned to work on 8-6 full duty.
Employee was dipping water and bending over and strained his back. He continued to work at his regular job and then started missing work on 8/30/2014 due to his back not getting any better.
Northwest 6 shovel hoist brake pedal came up abruptly as operator released the brake at high throttle pinning momentarily his leg between the brake pedal and the seat. Brake adjustments should be tested at idle before using.
He was pulling something out of a track-hoe and hurt his right elbow.
Initial injury date was 7/29/2014. Employee continued to work all scheduled shifts until 8/5/2014 when his MRI was read. Employee returned to work on 8/13/2014. Employee was hanging a vent tube when he felt a pain in his right arm.
Employee was securing a metal pipe when a hardened piece of material struck the opposite end of the pipe causing pipe to move upward hitting employee in the face.
Stepped on uneven ground and twisted right leg/knee.
Employee was moving several belt frames and felt pain in his groin.
Employee was carrying and spreading rock dust bags when she felt pain in her left shoulder. LTA effective 10/06/14.
Employee was unloading crib blocks off a flat car when he slipped and fell, and felt pain in his lower back.
Employee was getting off of a bobcat when his foot slipped and got caught between the arm and forks of the bobcat and twisted his foot. Employee worked until 8/5/14, went off work on 8/6/14.
Employee explained that left side drill steel was stopped up and he tried to assist left side pinner. When he entered the area to beat on the steel he went to duck under the light receptacle on the ATRS; he hit his head (hard hat) and it jarred his neck backward. Employee was under supported top at the time. Started losing time on 8/15/2014.
Employee stated he was climbing on track scaler to change an component. Upon climbing down the ladder to retrieve a tool as this time his right knee started to hurt. Employee said he tried to stretch his knee out and he felt a pop in the right knee. Doctor released employee back to full duty. He was referred to an Orthopedic clinic and on 10/30/2014 put on restricted duty.
EE began to trouble shoot an electrical problem at the plant & after visual inspection determined troubleshooting in MCC was necessary. He then opened the transformer for visual inspection. Using a T5-600V multi-meter to test voltage, the meter exploded causing a flash incident. The transformer is 4160V AC. He received 2nd and 3rd degree burns to both hands and forearms.
7/29/14 Employee was assisting with a screen change, after crew was finished-EE complained of lower back/hip pain. 7/31/14 EE was seen by physician and placed on restricted duty with no work activity allowed until cleared by phys. EE prescribed anti-inflammatory Rx, rest and ice for pain.
A PICI contract employee was working on the No. 29 Belt and slipped in the mud and twisted his left knee.
Employee bent down to pick up a bag of rock dust and felt a sharp pain in the butt cheek and leg.
Employee was conducting clean-up inside and around the clean coal chute of a screen machine and using a hoe to dislodge fine coal material build-up on the chute. Driller was in process of cutting air hose to splice, grinding wheel on cutter flew apart and sliced into chest of operator. Cut deep but apparently no penetration.

Employee was helping hang the miner cable and water line, he bent over and picked up the miner cable and felt discomfort in his groin area, he continued to work until 9-24-14 when he was taken off work and has been diagnosed with a hernia and is awaiting surgery to repair the hernia.
Miner was replacing a teeth on a excavator bucket. One tooth was stuck so miner used a hammer to loosen the tooth from the shank. When struck a small piece of the tooth broke off and struck the miners right forearm causing a puncture wound.
The crew was standing beside the crew pickup having their safety talk when employee fainted. As he was going down he was stepping towards the truck and twisted his ankle and he hit his head on pickup on the way down. As he came to he said he was ok but felt a little dizzy. Co-worker asked him about going to doctor and drove him in.
Contracted security guard was performing nightly walk around premises. He was walking between office & maintenance shop & tripped over the handicapped parking curb in front of the office falling onto the concrete. He sustained several broken bones. The light located on outside of maint. bldg was illuminated but light was blocked slightly by shop truck parked in front of bldg.
Employee was carrying a timber to be set on the 3 section pillar section. He felt pain in lower back area.
Employee stated that he was tramming the scoop outby on the #2 section roadway when he ran over a rock in the road. This caused him to bounce in the seat and land on his left hip, resulting in a strain. This incident became reportable when the employee began missing work on 8/4/2014.
Ee was driving haul truck underground, hit bump in the road, causing her hard hat to come off, hit the roof of the cab, and then hit her head. Laceration requiring stitches and put off work after diagnosis of concussion.
Employee reported that he strained his back while performing Jaw Crusher maintenance.
Employee reported that while cutting metal rail with cutting torch, piece of slag fell into his boot burning the front of his ankle.

Employee was using a shovel to clean the tunnel when he felt pain in his groin area.
Employee was in the copper underflow, slipped and caught herself without falling. She felt pain in her lower back and reported to her supervisor. She was taken to the ER and taken off work. Currently ongoing.
The miner was traveling on a downhill slope and lost control of tractor and collided into berm at the bottom of the slope resulting in his head striking the air conditioner vent inside the cab.

In 23D at 39 block 2 to 1 (track chute) the employee was on the right side of the Joy 14ED25 miner installing the last strap in the entry (after holing). After finishing the drill hole, he lowered the timber jack and a piece of roof rock (18""x12""x1"") fell and struck his right elbow causing small lacerations and contusions. No issue with mining methods, compliance, PPE or training.
Employee was walking off section when he came across a water hole. Not realizing there was a drop off in the water hole of at least 12"", he proceeded causing him to trip and twist resulting in a lower back strain. Doctor took him off work on 8/18/14.
Limited access to task location. While making adjustments to equipment employee stuck right elbow on steel crusher tire. Employee sustained a small puncture wound (.5mm) on elbow.

Forklift came into to remove pallet of product, causing the pallet to shift which resulted in the bags of product twisting the employee's left knee.
Employee was installing a eight foot roof bolt into a hole. when the miner pushed the bolt up he felt a pain in his abdomen. The employee did not start to lose time until 9-10-14. the following were not a factor: Rules, regulations, equipment, clothing, protective devices, attitude, skills or proficiency.
Employee stated that he was helping change the tire on the roof bolt machine. As the tire was removed, his left ring finger was caught between the rim of the tire and the hub of the roof bolt machine. This resulted in a fracture of the distal tuft of the left fourth digit.
Employee was exiting a diesel getman to tighten a ratchet strap, while he was pushing on the ratchet he felt pain in his groin area.
While hanging messenger wire for the high voltage cable, the employee claims that later near the end of his shift, he felt a pain in the left side of his neck. The employee missed 3 days, worked the next 2 days, missed the next 2 days, worked the next 2 days and has missed the last 3 for unknown reasons.
Trainee was looking for his cell phone and said he was going to the office to see if he left it there. He instead climbed back on top of the trailer without fall protection and hit his head on the overhead shoot causing him to fall off the trailer toward the ground. He landed on the drawbar between the trailers.
Initial injury occurred on 6/29/2004, when employee was operating a scraper water wagon. The employee drove through a rough spot in roadway causing pain in his neck. Employee is currently complaining of numbness in his hands caused by the injury in 2004 and is unable to return to work.

EE was in the process of setting up a longwall tripper drive @ x-cut 94 in entry #1 on Headgate 3. EE was jacking up an I-Beam to position it when the beam turned over onto his right foot. It broke 2 bones on the top of his foot.
Employee was loading belt structure when he felt pain in his lower back.
Employee was tearing down monorail in an abnormally high area. As he tried to free the chain from the support bracket he felt pain in his right shoulder. He continued to work all scheduled shifts up to & including 07/25/14. He sought treatment after his shift on the 25th because the pain began radiating into his neck. His first day missed was 07/28.
Injured employee was walking in the #2 entry of 7 Right outby the power center when he tripped and fell, fracturing his right leg.
Employee was walking to feeder on the section, when he felt a pop in his left knee.
Employee states he had been lifting wooden crane pads and back was strained
Injured suffered a ruptured eardrum while at work, initially injury was thought to have been caused from sinus pressure. On 8/4/2014 injured was seen by a specialist who determined the cause of the rupture to have been caused by a foreign body entering the ear.
Employee stated upon arriving to work he exited the cab of his truck and slipped on a rock falling to the ground. As he fell he extended his left arm to catch himself. This resulted in a radial head fracture of the left elbow. The employee has worked each scheduled shift at full duty, but on 7/28/14 he attended a doctor's appointment, which resulted in one lost work day.
Employee was lifting a belt top frame off of his golf cart, he felt pain in his back as he was lifting. At the request of MSHA, modified to 7 days before returned to work.
While attempting to tighten a fire valve, the employee incidentally activated the cut off valve causing a surge in pressure and the fire struck him in the right hand.
Employee was conducting his pre-shift inspection on Cat 777 haul. When he went to descend the truck using the ladder on the cab side, his foot slipped and he fell from the walkway. He suffered bruises to his left shoulder, left foot and left leg.
Employee stated that he completed performing maintenance work and was stepping off the feeder when his foot slipped, causing him to fall to the ground. This was a same level fall that resulted in a back strain.
EE was not feeling well before going underground. He rode in on his 4-wheeler, went to the feeder told the shuttle car operator he was not feeling well so he was going to leave to go home, but he would stop and check his drive on the way out. Said he didn't do anything to hurt himself but hurt worse as he rode.
Employee was getting his lunch box out of the pickup truck and got his foot stuck in the mud. When he went to leave he twisted his right knee.

When removing door on rotary ball mill, the door lodged in the opening and employee went to dislodge without relieving tension, the load released and employee was pulled into the opening sustaining trauma including bruising and 2 cracked ribs.

As Employee knelt down on his knees at the section phone he felt a sharp pain in his lower back. Conducted an internal investigation and determined this is a reportable incident.

On 8/21/2014 employee stated that on 7/20/2014 he was shoveling belt and felt pain in his back. The incident became reportable on 8/25/2014 when medical received confirmed the employee had sought medical treatment for a work related incident on 7/20/2014.

Employee was retrieving a rope and chain from under a rock that was on the ground. When the rock fell forward and caught the employee and pushed him to the ground. The rock ended up on his lower legs and back.
E/E was sprayed with sulfuric acid while working on the rail car unloading dock.
Employee was changing blade on D-9 dozer when he caught his left index finger between blade and frame smashing index finger. He was taken to Pineville Comm. ER room, finger is broken and he received stitches.

EMPLOYEE IS UNABLE TO RECALL THE FALL OR ANY CONDITIONS LEADING UP TO THE FALL. EMPLOYEE HAS 5 FRACTURED RIBS, BOTH LUNGS WERE PUNCTURED, 4 FRACTURED VERTEBRAE, AND A SPINAL CORD INJURY.
The IE traveled over a rough area and began to experience pain in the low back and neck. The IE was referred and seen by an orthopedic doctor who diagnosed a whiplash type injury.
Finishing belt lay struck by water line received 7 stitches.

The force of knocking the steel piece loose caused it to ricochet off the boom and struck another employee in the ribs on his right side.

he was working on the center packer when he need a part from the north end packer so he unplugged the 120 from the rec and opened the control box to right number down and felt a sharp pain in the right hand like he got a shock.

Injured employee stated that while operating a shuttle car, he struck a hole in the roadway causing pain in his back. Later in the shift while attempting to lift a miner cable he felt increased pain in his back. Employee was diagnosed with a strained lower back.
Employee strained his back as he lifted a rock from belt line walkway and put it on conveyor.
While pulling a pin from mechanical splice, employee heard a loud pop and felt a dislocation to his right shoulder. Also diagnosed with a bicep tear.
The employee attempted to climb onto the tailings pond conveyor belt, when he felt a pain on his left knee. The employee was taken to local hospital for treatment. The X-rays were negative.
On 7/17/2014 employee was loading jumbo steel onto rack in laydown when he experienced discomfort & some pain in his left shoulder. He went to medic on site -It was a report only. On 7/29/2014, he stated the he still had some pain in his left shoulder, went to doctor - restrictions were then issued. (first shift missed, 8/2/2014).
Employee stated he was doing repair work to the feeder on #4. He was cleaning the feeder flights and bolts holes out as he was laying them on the side frame of the feeder. At this point, one feeder flight slid off the feeder and struck the employee in the left lower.
Operator stated he had neck pain and numbness in his left arm.
Employee was starting a drill steel in the mine roof when a small loose rock fell. Employee tried to push rock away and smashed his left thumb against canopy post.
IE was helping hang ventilation curtain when a rock fell out between roof bolts and struck her on the lower left leg. She felt pain in her lower left leg and knee. She has a fracture to the lower left tibia.
Employee was helping install a boom on a miner when the chain broke and the chain hit the employee on the right foot.
While moving the section the employee bent over to pull on the miner cable and felt a sharp pain in his lower back. He went to the doctor and found out that he had a pulled muscle.
Employee was dismounting from D6R Caterpillar dozer when his left foot slipped between the track and the blade arm. He tumbled forward injuring his left knee and left wrist. Injuries were diagnosed as a sprained left knee and left wrist. The fall was caused by buildup of material on the dozer blade arm.
Employee was riding in man bus when the bus hit the inside heel flange of the track frog causing the bus to suddenly stop, straining employees back and neck.
Employee was walking to the fuel center when slipped and hurt his left shoulder.
Employee was in a haul truck and was being loaded by a shovel. A boulder in the dipper of the shovel shifted after it had been dumped into the haul truck, striking the front of the dump bed causing the truck to be shaken, injuring the haul truck driver.
High pressure hydraulic fitting failed on the shovel resulting in loss of bucket swing arm control. This happened while loading a haul truck and the bucket hit the rear and top of the head ache rack, causing the driver to be shaken up and the driver complaining about lower back pain.
While changing dozer ripper tooth, ee hit pin with 4 lb hammer and piece of metal embedded in left arm near elbow. Originally determined ok to not remove, but orthopedic surgeon recommended removal due to location of metal near joint. Surgery on 07/24/14. Ee full duty until that day.

Working in the drill core shack, employee was lifting core samples and strained his back. He worked the remainder of his shift on light duty and requested the next shift off. He returned to work the following shift.
College temp. employee was walking on an unfinished over-cast to get a sign that was hanging in the way of travel. As ee arrived at the end of over-cast ee lost footing, and fell causing a fracture to ankle.
Two employees was picking up a drawhead to install on a flatcar they were moving, as the other employee picked up his side of the drawhead it allowed it to roll over onto the injured employees right hand, causing a fracture. He was wearing a mechanic's glove.
Employee was operating a 21SC shuttle car when he hit a hole in the bottom (mine floor) causing him to bounce in the operator's seat causing injury to his lower back and tail bone.
Injured was carrying rock dust in 23D in crosscut 2-1 at 36.5 block, when a piece of head coal (4' long, 3' wide, 1"" to 8"" thick) fell and struck the employee knocking him to the ground. Miner sustained contusions to the top of his head and some neck pain. No problems with mining machines, methods, training, PPE, or compliance issues.
Installing a (5) Five foot roof bolt, bolt twisted causing victim to fall forward straining his lower back.
On 7/21/14 called in stating he was going to the Doctor because he had blister on the top of his feet, on 7/14/14 Employee had helped repair a caustic soda line and reported he had spilt the chemical on his boots, over the next few days he developed blisters on the top of his feet (Employee is a diabetic)
On #9 bottom EE was unloading shearer tow bar from battery cart into sleeper. EE heard and felt a pop in his LT elbow. EE was in discomfort and could not extend his arm at this time.
An employee was descending a platform ladder and slipped on the third step down causing him to twist his ankle. The employee resumed work without informing his supervisor of the incident. The employee had the ankle checked the next day and X-rays revealed that it was broken.
Employee was lifting some dunnage when he felt a pop in his right elbow. Employee had surgery 7/24/2014 and became a lost time.
Felt a burning in his left hip when he was stuck in the mud and twisted his body.

A (1) ton come-a-long chain link broke allowing the slack to be taken up quickly in the system. Employee was caught between rollers/pulley & belt

Injured was turning off the water sprays on longwall shearer when a rush of air pushed him down causing him to strike his knee on a shield toe. He was taken to an emergency room where x-rays proved negative.
Pulling on wrench-wrench slipped-put arm down to break fall-sprained wrist
Employee injured his left leg stepping down off the belt structure. As he came down, he felt a pop and could not put pressure on it.
Employee dismounted his drill and stepped onto a rock that rolled under his foot. He started falling, caught himself partially, but his left knee hit the ground. Seen at Golden Health and put off work pending an evaluation by an ortho.
A roll of conveyor belt accidentally became dislodged and rolled against the back of the employee's left foot resulting in multiple fractures of the foot. This incident did not involve any mining equipment, nor any aspect of compliance with rules or regulations. The employee failed to review his surroundings prior to moving the roll of belt.
Had waterline for miner over shoulder pulling up slack and struck a low hanging roof bolt with the top of his head. Later while using slate bar to pry conveyor chain around sprocket, felt pain in neck, between shoulder blades and upper back.
The miner was pulling the welding cables to the vibrator when the positive and negative wire joined and burned his left hand.
The wrench cable was tangled up on the boom of the Freightliner mechanic's truck. Employee climbed on the bumper of the truck to straighten the cable and lost his footing. He fell to the ground and injured his right shoulder and elbow. He became a lost time on 9/24/2014.
EE slipped and fell injuring his right shoulder, while pulling a barge pump to sure for repair at ramp 60 pond. His right rotator cuff was torn during the fall.
Employee was walking to the stacker belt after a heavy rain storm, there was an accumulation of water he tripped fell forward and hit his head on the metal frame, leaving a red mark, there was no excessive swelling, no loss of conscience.
Employee was spraying Zep 45 lubricant on a sticky valve when the spray defected under his safety glasses into his eye.
The accident did not involve: rules or regulations, mine equipment or mining systems, job skills, miner proficiency, training or attitude, or protective clothing, devices on equipment. The employee had a section of stone slide into his back as while cleaning other loose material from a gob pile. the employee suffered a contusion to his middle back as a result of the incident.
Repair of RS Conv 99% complete w/severe storm minutes away. 2 riggers supplied by Edwards, Inc on catwalk loading tools on deck of hyd. manlift supplied by Hanson Agg. 1 rigger thrust a heavy steel pinch bar on deck of manlift & it slid across deck, under entrance w/no kick plate, fell onto conv. carriage, glanced toward EE who threw rt arm up as shield, hit his arm & broke it.
Employee stated that he was in the process of installing a rib bolt when his drill steel became stuck in the rib. He stated that he grabbed onto the drill steel and with a jerking motion attempted to pull the steel out of the rib. When he did this he felt a pain between his shoulder blades and down into his left arm.
Employee was standing in an area that had been mined approx. 20 ft. high so an overcast could be built. A rock fell from where roof and rib line meet and struck him on the right side injuring his neck and shoulder.
Employee was getting ready for shift performing daily maintenance on D11N Dozer. He was carrying a 5 gallon oil can and slipped on dozer tracks causing him to try to catch himself without falling. He skinned his shin and pulled muscle or injury in neck/shoulder.
Employee was helping install top rollers for a new belt installation. Another employee dropped a top roller striking the injured employee on the right ring finger. The incident caused a fracture and laceration that reguired six sutures to repair.
Employee while assisting two other individuals in replacing a troughing idler, suffering injuries to his left wrist/hand. The employee's hand was pinned between the idler and the conveyor belt frame. The crew had managed to nearly upright the idler when a wrench used in process slipped off the idler causing it to fall on the injured limb, possible navicular fx.
Employee was helping to remove a belt clamp when he pinched his right pinky finger between the clamp and belt structure. The incident caused a laceration that required 5 sutures and surgery to repair.
While setting Heizman roof jacks, Employee stepped on a piece of pipe that had been cut off, causing Employee to fall injuring his left knee. Employee had not missed work until 8/6/14.
While employee was resting his arm on the miner, piece of rock fell hitting his right arm and thumb.
Employee was dismounting the chain conveyor area of an Auxier Welding shuttle car. His right foot became entangled in a chain flight, twisting his ankle.
Employee was tearing down flypads in the #7 entry 50'outby spad number 10757 to move to a new location as he was pulling on the flypads, the flyboard broke allowing a loose piece of drawrock that was above the flyboard to fall onto the ground hitting him on the left ankle resulting in a chipped bone.
Based on Medical records received on 7/16/2014, the employee has been taken off of work for medical treatment. While bolting in 2L section, employees bolt wrench became stuck on the bolt head. While attempting to pull the bolt wrench off, his hands slipped, causing him to hit his left hand off of the rock protector/canopy resulting in a fracture to his left middle finger.

Crusher cover plate was placed approximately 5 ft. from bottom rung of ladder. Employee came down the ladder and stepped backwards until coming in contact with crusher cover. Employee tripped and fell into the cover.
A cloud of dust was sent airborne by a gust of wind. Dust entered eyes of employee.
The injured was carrying crib blocks to level storage unit for the longwall. He bent over and picked up a crib blocked and when he turned he felt something pop in his right knee. He sustained a sprain or a strain.
Driver took load 2 Quarry hit bump, hit head on the roof. Adjusted seat max height. Hit brakes; Shoulder hit steering wheel-pain neck&left shoulder. Stated speed~5mph. Pain in right arm also. Taken to Clinic-Xrays & medications. Lift restrictions. Steering wheel flat&no tilt. Tried to simulate. Was suspect & 8/15 WC determined head & neck would be covered not shoulder.
Employee was removing a timber from the roadway when he twisted his left ankle.
EE was getting off 992 loader at end of shift when he said he hung the toe of his boot on the boot brush causing him to fall onto platform. Had lunch bucket in left hand and fell onto left shoulder.
Employee was using a pry bar in an attempt to remove the hub from a water truck when he experienced lower back pain. As a result, he began missing work on 9/9/14.
Employee was grabbing a 50 lb bag to weigh when his left index finger got stuck in a metal conveyor. He was treated at the WMC and received 5 stitches and released.
The accident did not involve: compliance with rules or regulations, mine equipment or mining systems, job skills, miner proficiency, training or attitude, or protective clothing, devices on equipment. The employee was struck in his upper back, by a section of rock while tightening crib. He received a contusion. Did not start to lose time until July 17, 2014.
Employee was trying to remove a shuttle car snub by himself. When he unhooked the snub from the anchor bolt the weight of the snub pulled his arm toward the ground injuring the muscles and tendons in his left upper arm.
Employee alleges he was lifting and pulling rocks out of the crusher and some rocks fell on him hurting his neck, left shoulder and hip. Employee just started losing time on 7/28/14.
EE states he was walking & stepped on pipe on floor in the walkway. The pipe rolled & he began falling. He caught his fall by grabbing on to a machine guard. Later in the day he felt back pain. He went to the onsite nurse. Ice/ pain pills given. The decision was made by SKF to allow employee to rest & returned to work 7/10/14. A doctor did not put EE out of work.
Uncoupling and moving a diesel powered 4"" water pump from the pit to shoot coal.
Employee was struck by a rock while rib bolting off of the miner. The rock struck his right leg. The employee worked all scheduled shift until being taken off of work on 7/30/2014.
EE repairing the A/C on a Cat 789C rock truck when he attempted to ascend the equipment ladder he felt a pull in his left shoulder, no medical attention was given & EE finished his shift. *EE worked until 8/20/14 when he began missing due to a non-work related illness. Upon release from his illness he was unable to RTW. It was determined to be related to the incident of 7/8/14.
Employee was working on the 001 MMU in the # 8 Heading, he finished installing his inside bolt and the right rib rolled out, a portion of the loose rib struck his right calf muscle. He refused an ambulance and was taken to the Logan ER and diagnosed with a hairline fracture to his fibula.
While grinding off paint on a suspended load, employee step back on an extension cord cap, lost his balance and fell on his right knee.

While using a right angle grinder on a flange that had been cut off. individual lost control of the grinder. Individual received a cut to his right forearm.
The employee was dropping ventilation from the roof for re-use. A piece of rib coal and roof rock fell behind the employee and knocked him down. The rib coal/roof rock was 2-foot thick 3-feet long and 2-feet high.
Employee was kneeling down removing rock out of the walkway, when he felt a pop in his knee. The employee returned to work, and worked until 7/20/14. He returned to the doctor with continued pain in his left knee. The doctor took him off work on 7/21/14.
Employee slipped while working in Prep Plant and complained of right knee pain. The employee has worked ever since and recently had an MRI which revealed several degenerative conditions. He went off work on 4/13/2016 for surgery on his knee. This is being reported as an Occupational Illness from repeated trauma.

Two employees lifting printer enclosure from a pallet into the back of a pickup truck with a topper. While placing the enclosure on the tailgate one employee felt a twinge in their back. Employee was diagnosed with tilted down S1 vertebrae. Employee is a support person and not an actual ""miner"".
Employee stated that while setting up the CM in the face of the #3 Entry, his right leg gave out causing him to fall and strike his right shoulder on the mine floor which caused pain in his right shoulder, back, right hip, and right leg.
Injured employee was attempting to get a D-ring on a shield free of mud and struck his finger between a steel pin and the shield base. Fractured right index finger.
Employee was running Tanks of dusters along the E Track @ E15 section. While he was checking on the #2 tank, he removed the cap and it blew his hard hat and glasses off causing dust to get in his right eye. He was diagnosed with a concussion.
Injured had tested a mine rescue apparatus (Draeger BG4) and placed it back in its container. When he picked up the container to place the unit back in storage he felt a sharp pain in his lower back. Diagnosed with strain/sprain of lower back. No problems with training, PPE, compliance issues, mining machines or methods.
Employee was using a wire rope and sheave block assembly to pull a piece of belt structure into an elevated installation position, when he got his hand caught in the moving cable and sheave block wheel, causing injury to his fingers.
Employee was replacing the cam gates when he hit his head on an overhead beam. The employee was wearing his hard hat at the time.
On July 15, 2014, the employee saw a Dr. and was not allowed to return to work due to an injury that occurred on July 5, 2014. On July 5, 2014 the employee was shoveling coal from a shuttle car with a broken conveyor chain. While shoveling, the employee struck a conveyor flight with the shovel and strained his left shoulder.
While cleaning a hole, the lip of the bucket caught on the rib on the way down, striking a rock, jarring the employee in the cab. He felt pain in his upper and lower back. He was examined by a physician and placed off work pending follow up.
IE was spotting in an additional roof bolt when a rock fell out between bolts and struck him on his right leg. He was originally treated for a sprained right ankle. We have been waiting on the results of his MRI and it has revealed a lisfranc fracture to his right foot.

Employee was attempting to mount the ladder on a CAT 740B haul truck. His right hand grabbed hold of rail with some grease on it. As he shifted his weight up on ladder his hand slipped and he fell from ladder. His weight came down on his left foot/ankle.
Employee stated that he was removing the bottom pin from the steering cylinder on the loader. As he was coming down off of the step ladder onto the ground, he stepped onto uneven ground causing him to sprain his right ankle. Missed two shifts of work.
While walking the employee stated that his foot got stuck in the mud. As he pulled his foot out, he twisted his left knee and felt a popping sensation and a sharp pain. The accident did not involve any aspect of compliance with rules or regulations, nor any mining equipment. The incident did not involve any personal protective equipment.
Employee was standing along side the miner when a piece of rock fell from the mine roof and struck him on the head, left shoulder, and left arm. It resulted in a contusion. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
Miner was fueling a D-10 Dozer. He was assisted by another employee. When he attempted to hand the fuel line back to the other employee, the miner lost his balance and fell off of the equipment and landed against the other employee.

Employee approached the extended boom forklift from the blind side to discuss job task with operator. After discussion employee turned to walk away at which time the employee's left leg was caught under right rear tire, pulling him to the ground partially running his leg over.
The associate was in the process of tying up a ship, the spring lines were being moved with Gantry 101. The associates hand was in the chocker strap that holds the rope, the strap tightened up when it got stuck behind the dock bumper which casued the chocker to tighten up around the associates hand.
Employee filling up the ball bucket when the balls got stuck in the charging chute. He attempted to get them unstuck by using his left hand, and as he started to dislodge them, the ball on top came loose and hit the tip of his left pinky finger. He was taken to the ER, diagnosed with a distal fracture and referred to the orthopedic clinic. Currently ongoing.
Employee operates a large dragline that utilizes a joystick for control and began noticing pain and numbness in his hand. Referred to a doctor 10/7/14 diagnosis is rupture tendon left hand.

Mechanic was lifting a tire jack onto the flat bed of a truck. He felt a pain in his left shoulder and returned to work. He did not report the injury until the following day. He was seen by a doctor on 7-31-14 and was given a full duty release. On 9-5-14 he was given a light duty release in response to an MRI indicating a torn rotator cuff.
While stripping lines on a stockpile, an employee twisted their ankle.
Employee was indoors mixing mortar in a bucket and felt pain in her left thumb. She had a previous injury from 2008 where pins were surgical placed in her thumb. The pins in her thumb had come out of place while mixing the mortar.

Employee reported back pain as a result of the air-ride suspension seat's air compressor not functioning at full capacity. Employee woke the following day experiencing intensified pain and did not report to work. He received medical attention the following day and was returned to work.
Employee was working on top of a floater platform that was pulled from the C/W ditch and sat on the ground to change out a 30 amp air cooled sealing water motor for the Z North #2 TLP. Employee finished installing the motor, stepped back and lost her balance and fell about 2.5' to the ground.

The miner was properly proficient and was properly trained with a good attitude. This incident did not involve any ppe or protective devices on the mine equipment.
Drove over a couple of ruts in the yard causing the loader to bounce fairly hard. Ruts have since been filled in.
Injured failed to maintain three points of contact while climbing down from a dozer. He slipped and fractured his left leg.

Employee was operating Kubota 174 and was exiting the leach pad via main truck ramp for break. As the employee descended the ramp, the Kubota lost control as the dynamic braking engaged too quickly on wet haul truck road overturning the Kubota on the driver side.

Employee was traveling up the decline when his batch truck hit a pothole which resulted in the employee hitting his head on the cab ceiling. Medical evaluation determined the employee experienced a cervical thoracic lumbar strain and was placed on restricted duty.
Employee was removing rock from the walkway and his right hand was struck by a piece of rock causing a hand fracture. Restricted Duty.
Employee was backing a dozer up and reported that he hit a chunk of rock, causing him to be jostled in the dozer and experience back pain. He was evaluated by a physician and taken off of work.

EE was removing a coupling guard and felt a pain in his lower back. EE went to the doctor and came back with no restriction, sent to therapy on 9/9/2014 Doctor put him on restrictive duty.
EE was riding #6 dry man lift (vertical belt conveyor) to 6PL sample port when the man lift jerked due to a vacuum hose temporarily caught between a man lift platform and the floor opening. The injury became reportable on July 17, 2014 when EE was placed on modified work duties by the treating physician.
EE was working on conveyor belt and belt hit his left thumb. His thumb was caught between ladder and belt crushing it.
Employee felt pain in his back when he stepped down in a hole in the mine floor. Employee started missing 10/24/14.
The float crew was unbolting 20"" pipe near the #11 Pit. The leaderman was walking along side of the pipeline toward the next flange and stepped on soft ground, fell, twisting his Right knee.

A construction worker was forming columns on the second floor of a new pre heater structure. He was located inside of the beam form. There was another worker on the beam side of the concrete form, when an iron worker stepped on the form causing it to tip over onto the carpenter causing the injury.
While walking-up the stairs, he felt a pop and pain to his left knee.
He was in a man lift trying to change a gate at the scalpering screen, when he said his back was hurting. he was sent to a medical dr. after complaining of his back hurting. He was at the chiropractor the night before for his back.
Employee was stepping down to ground level from a stationary piece of equipment in the mill. Slipped on loose ground and braced his fall with his left wrist resulting in a fracture. No equipment was damaged and employee sought medical attention the next day.
While employee was tramming a roof bolter from #3 to #2 entry, he engaged the cable reel take up on the roof bolter which caused the cable to pull a rope hanger attached to the cable from a rib bolt. The hook on the hanger struck the employee in the right eye causing an eye injury that required medical treatment and lost days. He was wearing eye protection.
While preparing a to use the forks on a backhoe bucket a fork was dropped and struck the employees foot causing a broken bone.
EE was retrieving a garden hose from the well pump house and was reaching for the water hose spigot when he encountered the snake. The snake struck at him, causing him to jump back to avoid being bitten. The employee landed awkwardly on his left leg and fell to the ground. Upon attempting to stand, he experienced severe pain in his left knee. The result was a patella fracture.
Employee was installing split steel and didn't know his glove was caught. Employee went to Good Samaritan Hospital sustaining a fracture to his finger which requires surgery and will be off work for an uncertain period of time.
Employee slipped while accessing the engine compartment causing pain in shoulder. On 8/1/20104 he was seen by a specialist giving him restrictions for work.
Employee was pre-occupied on a cell phone was not paying attention. The area that the worker was in was not a commonly traveled area. This area contained open holes that were not properly barricaded and protected. The worker stepped onto a piece of wood that was covering a hole that broke and worker fell with his left leg into the hole.

Employee finished his shift, went home, had dinner, spoke with his family, went to bed. Woke up with back pain sometime on 08/01/2014 and went to the emergency room. Was treated and released to return to work with restrictions.
He was removing a dump truck tire from the tire rack. While pulling the tire out of the rack he felt a pull in his left shoulder. This was last day of work week and was decided to see how shoulder felt the following week. On 8/5/14 he was sent to occupational health because shoulder was still bothering him. He was placed on light duty. 8/12/14 recheck sent to therapy.
Crew was stacking an I beam on top of another beam when one end slid off and pinched the employees finger between the beams. (Laceration requiring stitches)
EE was turning off water hose. Torn tendon in left thumb requiring surgical procedure to repair.
Employee spent 4 hours working around the fines discharge chute during shift. When employee got home that night, he had discomfort in his eyes, tried to flush them with no success. Employee sought medical treatment and had multiple microscopic particles removed from both eyes.
While escorting MSHA on E01 Inspection (Operating fan/airlock) opened airlock door, the inside door blew open and caused the first door to slam shut and smash middle finger of left hand causing a laceration.
While operating the roof bolter, the operators drill steels became stuck together in the roof. He tried to separate the two drill steels when he felt pain in his mid to lower back.
While lifting leg to step onto unit 14956, employee strained abdominal muscle.

EE was lowering an access ladder to board the fuel/oil truck and failed to move her fingers off the ladder before it was positioned against the truck. The EE smashed and fractured the right middle finger. She was taken for medical evaluation and placed on modified duty.

Emp. was pulling up a water line to relocate it, when he felt a pop in his shoulder. Pt denied need for doctor apt. until 8/18/14. Emp. taken to company doctor on 8/19/14 and placed on restrictions and an MRI was ordered.
EE was going to adjust AC and hurt his right index finger sticking it through the fan.
Employee was driving a Cat 777 Haul Truck and felt a twinge in his lower back. The twinge continued to get worse. Employee was taken to the clinic. Doctor said that employee has a lower back strain/spasm. Placed on restrictions.
Employee was pulling a trolley and felt discomfort in his upper back.
The employee was bolting when he experienced pain in the groin area and noticed what he referred to as swelling. He stated that he has had similar pain occasionally for several years both on and off the job but it was more significant than in the past. He was diagnosed with a bilateral hernia. He could not correlate the event to any specific activity.
Left index finger was smashed by the wear plate at the end of screen on the crusher.
Laborer on float crew was stepping over south matrix line with his left foot. As he applied weight to bring his right leg over, his left foot slipped in the sandy material and he felt his knee pop. He had immediate soreness in his left knee.
Employee was working in area 3100 doing normal job; he did not notice any discomfort until he was on his way home from his shift on the next morning. There is no specific incident that occurred which caused employee to get particles in his eye. When the discomfort in his right eye did not go away, he went to the eye doctor and had particles removed from his right eye.
The employee was working in a skid steer and finished the task he was assigned. When he was done, he stepped out of the skid steer and landed wrong on his left foot. He felt a sharp pain in the tendon above his left heel. The working area was clean and paved with no debris or abnormal working conditions.
Employee was using a pipe wrench to loosen a fitting. As he pushed hard on the wrench he felt a pop in his chest.

Employee's right foot slipped off of the lower grade support beam that he used as a first step to access the grader. Employee has a long history of chronic back issues. Employee returned to work on 8/4/14 with restrictions from his doctor.
Employee was examining work area when material above him fell on his face and got behind his safety glasses. A foreign body got lodged in his eye and could not be flushed out. Employee received medical treatment on 7/28/14.
While resetting tail-loop switch, employee slipped causing a strain to groin. He was seen by a doctor and released to full duty. On 8/13/2014 he was diagnosed with a hernia. He had surgery on 8/28/2014. Upon his next scheduled shift on 9/2/2014 employee was released to light duty.
After getting off of a ride the employee began to walking when he encountered rockdust and was having trouble seeing the bottom, he slipped on a piece of rock and twisted his right knee.
Employee was leaning forward to pass under a screw conveyor and felt a strain in his lower back. Conveyor is approximately 5 feet off the floor.

The employee was removing a 8 inch block, by pushing it with his foot, from under the front wheel of a 773 Haul Truck that had been loaded on a trailer.
EE was walking out of lunchroom when he lost his balance and twisted his right knee.
Employee was walking in bentonite unloading and stepped on an uneven portion of the walkway and rolled his left ankle.
Employee claims he felt back strain while lifitng sheet metal
EE was using a bobcat loader to stand up a pipe secured to the loader bucket. Once set the rigging was removed, EE was backing the loader away, the pipe tipped toward the loader, EE reached out to catch the pipe and the tip of his right little finger was trapped between the pipe and loader frame causing a small laceration requiring stitches and fracture to the tip of the finger.
Employee stepped on a rock and twisted knee. He believed it was fine at the time but recently went to the doctor back in Texas where he lives and is having his knee operated on December 20, 2014.
EE was removing an oil drain plug on a gear box when his allen wrench broke and he fell backwards into a grease fitting which struck him in his back just below the right shoulder. EE was diagnosed with a contusion/ myalgia and released to work with restrictions.

Employee was building scaffolds in area 45 bottom side and went to break afterwards noticing soreness in his lower right side of his back. Employee stood up from his chair after morning break to return to scaffold building and felt more significant pain to his lower back. Lower back strain. Temporary job duty 7/23/14 till 8/18/14
Employee was walking from back door of the plant to the front parking lot to get oil. He had stopped to scratch his head and turned into the belt structure beam and put a scratch on his head, on 7/25/14 he went to ldrndman clinic because he had a head-ach and was released.
EE was checking room for loose rock. He started to pull a loose rock when the one above it fell hitting EE on the left hand and arm.
Employee states that he was at the inby corner of the intersection in number 5 entry inby the feeder dump point when the corner of the rib rolled out and struck his right foot causing the steel toe of the boot to mash his toes causing the toe nail of the toe beside his little toe to loosen. The nail had to be removed.
Employee was placing rock on roller feed table and a sharp edge of the rock lacerated his finger.
EE & co-worker were lifting the bearing assembly into place. Not able to fit it, they were bringing back into the manlift when the flange caught on the top railing. Assembly came to rest on EE's right forearm pinching it between the top rail and assembly.

Employee tripped while coming down stairs, hurting her right ankle
Employee saw a doctor for discomfort in his left eye on 7/26/14, employee thought it was from dirt getting in his eye on 7/23/14. He was diagnosed with ultraviolet keratitis (eye sunburn) in left eye. After investigating we believe that it was caused by torch cutting without shaded eye protection on 7/20/14. Doctor restricted employee from welding for one week.
Employee stepped off a concrete step onto an uneven concrete pad and sprained his left ankle.
The injured employee was replacing the seals on the small pod. As he was positioning the west end housing back into place, a small amount of caustic cleaning solution got into his left boot causing a chemical burn to his lower leg and ankle area. (Deemed reportable on 8/19/2014)

Employee was working in the undergrate compartment of #2 cooler. Access to this area is thru a 4' square door. When employee entered the compartment, as he stepped forward and stood up, he hit his head on an X brace. He was wearing his hardhat, but felt a ""pinch"" in his neck. EE was taken to Dr.,who placed him on restricted duty (no lift,push,pull>10lbs)until recheck on 7/28/14

Employee was driving a forklift down a road when he hit a pothole causing a strain to his right side abdomen.
Employee was using a prybar to assist in moving a gate handle. The prybar slipped causing a contusion and fracture to the left thumb.
Employee informed that he felt ""a little stiff"" in the morning. He did not attribute it to any particular event at this time. We logged the incident in the first aid register at that time and followed up with him later that Friday. However on Tuesday morning 07/22/2014 he complained again of stiffness in his back. He was taken to a clinic. He was diagnosed with a strain back.
