Accident type, without injuries

Upon arrival at the mine site, the Assistant Plant Manager noticed what looked to be a transformer fire on the mine property. All appropriate notifications were made as soon as possible.

An unintentional roof fall occurred in F26 Longwall section at the tailgate @ approximately 86+67 to 86+40. The fall is approximately 27-30' long, 16' wide and 15' high. There was no injuries or equipment damage as a result of this incident.

At 11:45 pm an employee noticed a hydraulic leak and found a crack in the hydraulic block of the break unit of the #1 hoist. So, the hoist went down until the hydraulic block was replaced by a new one. This accident has been reported to the manufacturer and investigation will continue in order to find what caused the crack to appear.

A roof fall occurred in the 00 West Crossover air course. The fall is located in #2 Entry at 3xc. The fall is approximately 80 feet in length X 16 feet in width and is approximately 12 in height (two feet below the anchor point). The cause of the fall was determined to be weathering of the roof.

Roof fall occurred on the 5 seam air course. Location is 750W 525S. An action plan was submitted and approved by MSHA to install EP's at approved locations. The fall will be referenced as 33A

A Foreman called outside to verify the Intake phone was operating. The phone would not work so the employee walked the intake to find the problem. The employee found a roof fall at x-cut 26 in the #8 entry on the North East Mains #2belt. The fall was 4' to 6' High x 17'4 ' wide x 30' long.

A roof fall was discovered on this date. The fall was located on the 3-North-1 Mains at crosscut #9 in the number 4 entry. The fall measured 16' by 18' and was 5-7' in thickness. The fall was originally supported with 5' hi-tech bolts and 8' cable bolts. The primary escape way was rerouted and all employees were notified of these changes.

At approximately 7:15 a.m., the Spice Lick Elevator was taken out of service due to an issue with a bearing.

8 miners got on Metz elevator at the bottom. They shut the inner door, but did not shut the outer door and hit the call up button. This shut the elevator down for a latch switch fault. They were trapped until another miner shut the outer door and reset the fault at 12:36 am. No problems with mining methods, PPE, training or compliance issues.

Unplanned roof fall discovered reportable at 29 blk intersection of 4 belt. Fall approx. 20' wide, 9' high and 50' long. Occurred due to gradual deterioration of northern crosscut, and moisture infiltration of the strata, which rode out into the belt entry. No problems with mining machines, PPE, training or compliance issues.

A unplanned dome shaped roof fall occurred above the anchorage in the #4 entry of the non-producing East Mains #3 section at 19 wall. The fall measured 26 ft. x 18 ft. by up to 7 ft. in height.

#2 Service hoist went down at 2:30 am on 8/27/16 due to a burnt coil. Electrical was called and coil was replaced. Hoist was placed back into service at 4:47 am on 8/27/16.

A roof fall occurred in the 00 West aircourse. The fall is approximately 80' long in #2 Entry X 50' wide in 6xc X 15-16' in height. The area was originally supported with 8' resin assisted roof bolts. Weathering of the roof was determined to be the cause of the accident.

Grass fire located approx. ¼ mile south of the South West Pit, no mine personnel, equipment or facilities were at risk at any time. Suspected cause of fire deemed unknown by the CCFD. Fire was extinguished approximately 2 hours after it was detected. CCFD posted a fire watch for one hour after extinguished.

An unintentional roof fall occurred in the intake at break no. 28 in the no. 7 entry. There were no injuries as a result of this incident.

A roof fall occurred above the anchorage point on Main East Track at 738 crosscut. Fall was approx 25' w, 20' l and 7' h. Cause of fall was due to weathering of roof along the rib. To prevent further falls in this area we plan to add supplemental roof support and spray some areas with a strength enhancing cementitious material no other part 50 criteria apply

IGNITION - Occured on the No. 6 section in the No. 3-2 cross cut. The Continuous Miner was cutting bottom when a bleeder in the mine floor was ignited, the flame lasted 30 seconds or less and was extinguished with water from wash down hose.

A roof fall was discovered at crosscut 17-13 between the track and the belt. The fall is approximately 10' high, 16' wide and 30' long. No haulage or ventilation was affected. All accesses have been dangered off.

A roof fall occurred in the old 1 South Pass way. The area is in common with the track. The fall is approximately 100' in length X 16' in width and 14'-16' in height. The area was originally bolted with 8' resin assisted roof bolts. Weathering of the roof was determined to be the cause of the accident.

Machine was working, loading hay, trucks in pit #51 when a truck driver observed fire dripping from the belly pan, causing was determined to be a ruptured fuel line.

On 8-22-16 a roof fall occurred 8 to 10 feet above the anchorage point at survey station 31+25. The fall was approximately 16' wide and 25' long and approximately 8' to 10' high. The fall occurred due to deterioration of the mine roof. Supplemental bolts will be installed in areas that show deterioration. No other part 50 criteria applies.

Employee was testing the emergency hoist when the hoist went down for a gate switch fault. All employees were evacuated immediately and no persons were trapped or injured. The hoist was back up and running at 5:00 p.m. on 8-21-16

On Sunday 8-21-16 at approximately 9:10pm a fireboss examiner discovered a roof fall on the 2 left section. The fall was located in the face and intersection of the #6 entry 1 break inby survey spad number 7382. The fall measured approximately 19' wide X 60' Long X 7.5' thick

While climbing aboard a Euclid CH-150, the employee stumbled to the ground when one of the rubber straps for the boarding steps broke. Decker Coal is filing this form under protest, as no injury resulted from this incident. Decker Coal has to file this 7000-1 form in order to abate a citation resulting from a miners complaint on 12/20/2016.

Mine power had been off for several days due to lightning strike, no fan also. After fan was restored the mine was being firebossed and a roof fall was discovered at 10:05 am in the intersection of #6 entry @ x-cut # 76 on the Main West.

A unplanned dome shaped roof fall occurred above the anchorage on the 8 Right development section at 81 wall in the #2 entry. The fall measured 16 ft. long by 20 ft. wide by up to 7 ft. above normal mining height.

On 8-19-16 a roof fall occurred 8 to 10 feet above the anchorage point at survey station 14+50. The fall was approximately 16' wide and 15' long and approximately 8' to 10' high. The fall occurred due to deterioration of the mine roof. Supplemental bolts will be installed in areas that show deterioration. No other part 50 criteria applies.

A roof fall was discovered at 3:20 p.m. between crosscuts #30 and #31 of the 5 North Left Side Return. The fall is approximately 100'long, 10'high, and 14'wide. The area in posted of prohibiting travel, however ventilation has not been disturbed.

At approximately 5:55 am, it was found that the Uninterruptible power supply (UPS) on our #1 secondary escape shaft was not working causing communication issues. It took approximately 9 hours for us to identify the cause of the issues and return the hoist to normal operation.

The E skip was positioned approximately 165' about the skip pocket load light. The skip was positioned at this pint to allow it to accelerate to full speed in order to troubleshoot the voltage output from the tack generator. The hoist operator was instructed to bring it to full speed. EE selected wrong direction and the W skip was at full speed causing it to go to bottom of deck.

An unintentional roof fall occurred in E27 Longwall section at 4+18 in the #4 belt entry. The fall measured approx. 16ft wide by 25ft long and 16' high. There was no injuries or equipment damage as a result of this incident.

Roof fall 5th main west entry 3 XC5 in the neutral beside belt entry. Area was bolted with 8ft TR and 12 foot cable bolts and had some supplemental support extra bolts, straps, Area was in a fault zone, ventilation was not affected. Stoppings were intact, area is dangered off on all sides and will not be cleaned up. Size 20ftX30FtX10Ft high.

A roof fall occurred above the anchorage in the West 8 heading, from the North 5 intersection to the North 6 intersection. There were no injuries due to the fall.

ROOF FALL - Occurred in the No.4 entry on 6 section of the E9 tailgate panel. The fall was approx. 28 feet in length and 5-1/2 to 6 feet in height up to the main roof.

The hoist operator was doing the pre-shift test trip and was getting incorrect slack rope faults on the load cells. Once on site the engineer investigated and found a loose wire in the control box of the load cells that caused it to give incorrect faults. Once they tightened the loose wire and re-calibrated it everything worked as it should.

A roof fall was discovered at 7-36 crosscut between the belt and the track. The fall is approximately 18'long, 20'high and 16'wide. No ventilation or travel way was affected. All accesses have been posted off.

An unplanned roof fall that impedes travel of the track / secondary escape way occurred at 25 crosscut on the 4 South track. The width of the fall was 45 feet wide. The length of the fall was 20 feet long. The thickest point of the fall was 2 1/2 feet thick.

Abandoned farmhouse at entrance to mine property was set on fire / arson. Fire was not the cause of mining operations. No miners were at risk as plant was shut down for weekend - no shift at work. Building had no electricity or active heating sources. Fire Marshal investigated scene on 08/14/16 and indicated suspicious cause - most likely arson.

There was an unplanned roof fall on the 1-54 Supply Road at x-30 to x-31, spad number 21+60 to 22+40.

A roof fall was discovered in the secondary escapeway, #4 entry of #4 Mains, 140' inby spad 1462, as the fire boss was making pre-shift examination. The fall was approximately 6' high, 20' wide and 30' long.

A roof fall was discovered in crosscut 13-33 1/2. The fall is approximately 12' long, 10' high and 14' wide. No ventilation or travel ways were affected. The area is approximately 5 miles from active workings.

Roof fall discovered 10 belt 77 block, 6' high, 25' wide and 60' long (approximate). Fell on top of old, unused, overcast, knocking it down on top of belt. Re-supported with 8 and 12' bolts, screen and floor to roof supports. Fall occurred due to obscured deterioration of slicken sided clay material. No problems with training, ppe, mining methods or compliance issues.

Employee was testing the emergency hoist with inspectors when the hoist went down for a slack rope sesor issue and the gatelock at the bottom of the shaft. All employees were evacuated immediately and no persons were trapped or injured. Hoist is back up and running

A NON-INJURY ROOF FALL FELL IN THE INTERSECTION OF NUMBER TWO HEADING (SPAD#1012) AND EXTENDED FOR 50 FEET TOWARDS NUMBER ONE HEADING. FALL WAS APPROX. 6.5 FEET HIGH. NO EQUIPMENT WAS INVOLVED.

An unplanned roof fall occurred in the #9 coal seam in 1st Northwest Submain Secondary between crosscuts #10 and #11 in the #3 entry. No injuries or ventilation short circuit occurred as a result of the roof fall.

A fire started in the G pump power switch in the substation on 5300 Level. The main breaker tripped thus putting out the fire. The mine was cleared of gases and returned to normal mining activity.

An electrical malfunction in the slope car control system caused a fuse to blow. Electricians were dispatched immediately to resolve the issue. The mine was not producing at the time. Only 10 employees were underground at the time and they were evacuated until the fuse was replaced at 3:00pm.

Employee was testing the emergency hoist with an MSHA inspector when the hoist went down. After troubleshooting it was determined that a lightening arrestor had went bad. All employees were evacuated from the mine immediately and no persons were trapped or injured.

A roof fall was found on this date during weekly exams. The fall was located on the 2-North Mains at crosscut #50 in the #6(Intake) entry. The fall measured 20' by 20' and was 5' in thickness. The area was originally supported with 36"" resin bolts and timbers. All persons were moved outby until the escape way was re-routed. No persons were affected.

Lightning storm passing through the area struck hay bales in mine reclamation area. MSHA, fire department, and shift supervisor were notified. Fire department plowed fire lanes and extinguished the fire.

Operator noticed flames from engine compartment and attempted to extinguish with portable fire extinguisher. When it was empty, operator moved clear & initiated emergency response procedures. Local Fire Dept was able to extinguish fire. Fire was contained to engine compartment & likely caused from battery cable arcing to hydraulic hose at frame rail.

An unplanned roof fall occurred in the #9 coal seam in Panel 22 at crosscut 117 in the #6 (Belt) entry. No injuries or ventilation short circuit occurred as a result of the roof fall.

During a post blast inspection of the 215 level, a fall of ground was identified in the 1485 stope. The ground failed to an unidentifiable contact in excess of 20 feet above the back of the stope. The area was supported in accordance to the ground control management plan.

Contractor was cutting hay in D3 area when the hay cutter contacted a rock causing sparks igniting the grass. The fire burned approximately 20 acres before being extinguished by water truck and Texas forestry department. MSHA hotline was called at 1:25pm. The fire burned for approximately 3 hours and 35 minutes.

A roof fall was discovered in the 13-17 crosscut between the track and the belt. The fall is approximately 10' high, 15' long, and 16' wide. No ventilation or travel way was affected. Both sides have been posted and dangered off. The area is approximately 5 miles from active mining.

An unintentional roof fall has occurred on the M-2 at the 4th XC of the #4 entry, the fall was approximately 80' in length by 17 3/4 wide by approximately 14' high.

Divider wall in the shaft between the south skip compartment and the ventilation compartment shifted into the ventilation compartment, creating an offset in a guide splice. The south skip impacted the offset guide breaking the guide and then impacting the guide and divider timbers on a subsequent trip.

Fall was about 8' above the anchorage point and 11 by 15' wide at the intersection to 4 north.The cause of the fall was due to weathering of roof.Supplemental bolting is being done in these areas.There were no compliance issues and no other part 50 criteria applies

A fall of the roof occurred on the 1st N.E. Sub-Mains, 2A Return, #9 entry, crosscut #10 (intersection). The fall measured approximately 50' length, 15'-17' width, & 18'-19' height.

A fire occurred in the coal storage silo feeder area, above the Unit 410 Belt Feeder. No persons were injured during this event.

A brush fire was reported near the red dog corner around 4:05 pm. I reported the fire to MSHA at 4:27 pm. Fire was out at 6:00 pm. No injuries or equipment damage occurred due to this fire. Vegetation conditions in the county were very dry.

At approx. 5:15 AM surface maintenance discovered that the PLC system for the production hoist had frozen preventing them from being able to operate the hoist controls. Trouble shooting was done, the PLC was reset, and the hoist was functional.

Door did not close completely on the north skip on a prior trip, contacted the shaft collar and broke the door arms. Subsequent trip, the broken door would not close at all at the dump and hung the skip in the headframe. Mgmt became aware of the skip damage at 7:22am.

An unplanned roof fall occurred in an out by area of the mine at # 102 to # 103 block haulage road. There were no injuries. There was no equipment damage. There were no escape ways compromised. There was no disruption of mine ventilation. This event was immediately reported to MSHA.

The couplers on the slope car were found out of gauge and the slope car was taken out of service to make repairs. The repairs were completed on 08/02/2016 at 11:30 am.

While mining on a working section a water inundation occurred at the face. No injuries or equipment damage occurred. No escape ways were compromised. The section equipment was moved out by. Dewatering pumps were installed to remove the accumulation of water and control the inundation.

Struck by falling object

Employee was changing a cable pulley that raised and lowered a load spout. While installing the new pulley, something broke. The cable struck the employee in the head area fatally injuring the employee. The investigation is still ongoing to determine the exact cause.

Caught in, under or between a moving and a stationary object

Employee was removing a choked pipe from the hopper with co-worker, as the co-worker removed the last bolt, the pipe twisted and caught left pinky finger between pipe and silo, partially severing the tip, which required surgical removal.

EE stated they lost balance on workboat causing them to try and catch themselves. When their hand was placed on the deck, it apparently was underneath the mooring rope and the workboat deck potentially causing the rope to crush EE's right hand. *EE's middle finger was removed during surgery.

Miner reported on 8/21/16 that they were moving a heavy object on their own on 8/20/16 when they felt a pain in their abdomen. There were no witnesses to this happening and miner told no one of it until the following day. Miner went for evaluation on 8/22/16 at a nearby occupational clinic. The doctor then diagnosed miner with an umbilical hernia.

Rolling rag and it caused a blister on toe. The blister caused toe nail to fall off and become infected. The employee is a diabetic. The accident report wasn't filed with the company until 2 weeks later due to the strange circumstances.

An employee using an ironworker sustained a finger injury. Taken to ER for treatment. Has been taken off work.

Removing bolts from a valve when finger got caught between wrench and pipe flange taking off tip of finger.

Mechanic received a crush injury to right thumb resulting in amputation of right thumb nail bed to tip of thumb. Mechanic was lowering a 350 hp electric motor to motor base utilizing an electric crane hoist and had thumb positioned in the ""line of fire."" Electric crane performed as designed.

Employee lifted a bag of rock dust to fill a can duster when they felt a pain in lower back. LTA 10/26/16

Injured employee felt pain in EE's left shoulder as EE worked to maneuver the ansul rig hose over rock next to the left rib while loading and underground shot

Struck by falling object

Employee was dismantling a platform assembly when one member of the assembly fell and struck EE's leg. The injured employee was treated and taken to a local hospital.

Employee stated ee was stepping into the deck of shuttle car when ee felt a sharp pain in right inner thigh. This resulted in a strain and lost work days.

Employee was carrying a 42"" top roller. EE slipped on the track and strained a muscle in EE's chest trying to hold on to the roller.

Over-exertion in lifting objects

Employee stated that ee was loading a hydraulic pump onto scoop 515 at the end of track on 1 section. Employee stated that ee back started hurting. Employee finished shift returned to work schedule shift. On 9-7-2016 Employee sought medical attention and was taken off work. On 9-8-2016 this became a lost time injury due to missing work.

Struck against stationary object

Employee struck their head on scaffolding pipe that crossed near the top of the steps to C3 belt. Employee did not feel pain at that time and reported incident on 9/6/2016.

Over-exertion in lifting objects

While carrying rollers into belt line, through a man door @ 92 block, 5 West, employee felt pain shoot through lower back, and down right leg.

Fall onto or against objects

Employee was standing at #3 shield on E27 longwall face, when the face chain started and a 3/8"" wire rope tightened and knocked employee to the ground and hitting left shin on headgate.

Over-exertion in lifting objects

While EE was bending over to pick up water hose they strained their lower back. EE worked 8/31/16 and became a RLT after seeking medical attention on 9/1/16. Lack of proper lifting technique was the cause of this incident. PPE was not a factor, mining equipment/mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.

Struck by rolling or sliding object

Rib rolled out between two straps hitting the individual in the upper Rt. thigh.

Over-exertion in lifting objects

Employee stated; was unloading 20ft sections of 4 inch fresh water line on #2 unit. Employee was working alone when they stated that they felt pain in their lower back on the left side.

Employee was putting up flypad boards & hit the wrong lever on the roof bolter causing the canopy to hit employee on the right side of head.

Employee was oiling a shuttle car cable reel chain and the cable reel cover fell shut slicing left thumb.

Stepping down off of #3 Tail when stepped on a block and twisted rt. knee.

EE experienced a sudden stop while scooping roofing granules with a skid steer from the rail loading area. The sudden stop caused EE to be restrained by the lap bar. This sudden restraint eventually caused soreness in the rib area.

Over-exertion in lifting objects

Employee was helping another employee load a top belt roller into a scoop insert and strained back. EE started missing work on 8/30/2016.

Over-exertion in wielding or throwing objects

Employee was swinging a hammer and hurt back. The initial reporting of the injury was employee stood up and took a deep breath and back hurt. This injury was found compensable 9/9/2016.

Fall to the walkway or working surface

Employee tripped on raised concrete pad and fell to knees. Over time employee felt pain in right knee. A right medial meniscus tear was discovered, and surgery was performed on 3/21/2017. Lost time began 3/22/2017.

Struck against stationary object

Injured was walking through roof bolter 47, a platform bolter, when the injured tripped and jammed right thumb on a handrail.

Driving track pin with sledge hammer, sliver of metal from pin entered knee joint

Employee was cleaning mirrors on truck and allegedly fell.

Employee stepped out of the shuttle car that EE was operating. EE slipped and felt pain in lower back.

Over-exertion in wielding or throwing objects

employee stated EE felt a pop in EE's back when using a sledge hammer to clear product from a blocked feeder.

Flash burns (electric)

A cable was shorted that created an arc flash.

Struck against a moving object

Watering roads, going up hill the truck stalled out and began to roll backwards, employee turned it into berm to stop it and the loaded water truck overturned.

An employee was changing a hydraulic line and didn't release the pressure before taking off the clamp. EE got hit with pressured hydraulic oil in wrist and hand. There was not a cut or puncture wound to the employee. The employee was taken to the hospital to be checked out. EE was kept overnight for observation.

Employee was lifting rock off of a table to place on a pallet for packaging.

Riding rail car and hit head on bulkhead/ C/o pain, numbness and tingling in arm

Employee claims seat bottomed out driving over transition from dump to haul road. Employee was taken to the ER, and given a full release to next shift. On 9/12/16, company received medical documentation removing employee from work due to event as of 9/1/16. Currently Ongoing.

Fall from machine

Employee was changing a conveyor belt & coming down- foot slipped, body slipped, right foot got hung & employee fell approximately 1' to the ground

A mechanic was installing a pull block on the rear of a scraper. Employee got finger pinched between the pull block and a backup alarm mounting plate. The employee fractured the tip of right index finger.

3 individuals were in the process of replacing the lid on the power center. The injured person was on one side of the lid and the other two were on the opposite side of the lid. As they were placing the lid in place two of them accidentally dropped their end. The injured employees hand was underneath allowing the lid to land on left index finger.

Employee was drilling with a cobra drill on 7-38xc roof fall when the drill steel became stuck in the rock. While pulling on drill steel employee felt pain in back. No other part 50 criteria apply.

Employee reported strain in right shoulder after clean up work. Employee did not want assessment by site responder. Employee had surgery on shoulder 11-28-16.

Employee was working in an electrical panel with live 480 volt feed coming into unit. Employee was working alone, with minimal PPE. Employee did not try out or test circuit for current before beginning work. Employee was exposed to arc flash in close proximity to panel, experience burns on face, ear, left arm and hand.

Filing because of changes found that make it a recordable if not denied.

Employee was changing a fitting on face shield using a socket and ratchet. The socket broke causing employee to strike elbow on the adjacent shield. The employee continued to work until April 20, 2017 at which time a physician applied a cast to immobilize the elbow to aid healing which makes this incident reportable. The employee continues to work under restrictive duties.

EE just finished roof bolting. EE backed the roof bolter out of the heading. EE was standing by the center of the machine when a piece of draw rock fell between the roof bolts 4' thick 1' wide 2 1/2' length. Striking ee on right side of hand cutting index finger, 43 stitches.

Two employees engaged in a physical altercation that was unrelated to work. The incident occurred before the shift started, one employee pushed another down the office steps and landed on top of other ee. The fall broke one employees two ribs. The employee with the broken ribs was then struck in the face by the other.

Employee was helping move a pump, as ee stepped over a 8"" Aquamine pipe ee slipped in mud causing a fall and tear a muscle in left upper leg.

Worker was cleaning the waste material out of the chopper. When ee accidentally stepped on the operating lever. The workers hand and wrist was caught between the chopper teeth.

Employee was using an insert puller to remove an insert on the miner. Part of the puller broke and came back and struck employee on the left ear. Employee received 20 stitches on the left ear.

This fractured the ee arm is several places as well as lacerating it. First aid was administered on site, emergency services were called and ee was taken to the hospital.

While lugging on a tire, tech was using crane hands to line up tire with bolt holes. Guiding tire into place ee applied down pressure on tire. The tire slipped in the loose gravel causing the boom to surge forward hitting the tech in back, causing ee to lose breath and pain in the lower back.

Iron worker was cutting apart an old discharge chute with a cutting torch. When the chute section was cut apart it shifted causing the employee to lose EE's balance and strike EE's left wrist against a sharp piece of metal. EE received a laceration to the top of EE's left wrist and ulna tendon which required surgery to repair.

IE was driving a coal hauler through an airlock door when coal hauler hit the equipment door causing damage & loosening it. IE stated that while trying to push the inby door back into place IE felt a sharp pain in the back of the right calf. Employee continued to work until the results of a MRI revealed a torn Achilles tendon. The employee was taken off work on 9/29/16.

Employee was in the process of setting up a light plant when a chunk of mud fell from the light fixture striking the employee on the left side of the face between the hard hat and safety glasses. Mud entered the left eye behind the safety glasses.

EE was standing on the ladder when they suddenly fell backward onto the mine floor. The employee did not slip, nor did the ladder tip over. EE was side bolting in E29 installing a new roll of mesh on the miner. EE had to lower strap rack down in order to move the roll past the roof strap already on the miner. As ee stepped down off the miner, the miner sumped forward and the bolt tray hit the strap and caused it to go outby. The roof strap hit the employee behind left ear and on the back of neck.

The plant was shut down and equipment locked out. Hazmag impact crusher designated as cr1 to do maintenance and the door was opened too far and lid came down and the guard attached to the lid struck the employee in the arm and face. Resulting in fractured cheek bone.

The driver was trimming EE's load on an end dump trailer. EE closed the tailgate using the in cab controls and then went to the back tailgate to clear the seal so the tailgate could shut. As EE grabbed the horizontal grab bar below the window and started pulling self up further, EE's gloved slipped off causing EE to fall to the ground landing on EE's feet. When troubling shooting a limit switch, on a load out chute and is 25 feet above ground level. The electrician was utilizing mobile scaffolding to access the limit switch. The electrician adjusted the switch and raise the chute. When the switch was activated, the chute began to rise and struck the scaffolding. EE fell, struck the scaffolding and fell on concrete floor.

A track hoe slack adjuster coil spring was being cut for scrap, during the process, a second employee (victim) was walking in the vicinity of the work. The center pin (inner portion of the adjuster) broke during the heating of the coil spring (outer portion) of the adjuster and struck the victim in the lower left leg while walking past, fracturing leg above the ankle.

Miner was mounting dozer for beginning of work day. While standing on the track of the dozer, the miners foot slid off of the track causing the miner to fall, striking lower left back on the bottom step of the dozer. Miner was taken to ER and was diagnosed with a Lumbar Transverse Fracture.

As a dozer operator was dismounting dozer 327 at the end of EE's shift, EE failed to use the workable power ladder installed and used the iron grab irons to dismount. When EE got to the left push arm, EE jumped to the hard ground where there were small baseball size rocks. Upon landing EE twisted EE's left ankle and twisted EE's left knee falling on EE's right shoulder.

After placing the first pin EE was drilling EE's second hole and the drill steel stopped up, the EE went to the middle of the bolter to retrieve a new drill steel, when a rock from the rib fell down, an hit the wire mesh that was leaned up next to the rib and hit EE in the back of the right leg.

EE was driving a 773B 60 ton rode truck. EE primary duty was to take loads of material ie, rock, clay, soil from pit to storage area at storage area EE misjudged the berm and ran the pickup. Driver side truck remained level this shifted the load and threw the truck down on the driver side. EE went to the hospital by ambulance. Was given 3 days off by the hospital.

Employee was trouble shooting the air system on the vactor truck when they reached into a pinch point area and a butterfly valve shut on their left index finger.

On 5.1.17 an email from the company outside attorney came claiming the illness is a possible work related illness or aggravation of an illness. EE is currently on restricted duty. A Cat EX1 excavator was loading a rock into the bed of a Cat 777 when the rock dropped out of the bucket and struck the truck bed. This action caused the bed to raise slightly and then came down jarring the operator in the cab. The truck driver complained 4 hours later of a pain in their back. The employee was examined by a physician and diagnosed with a low back strain.

Employee completed the task of replacing drive belts on the #1 attrition cell. Employee, along with the Mill Operator and the Mill Helper were installing the belt guard when the employee felt a pull in lower back.

Stepped off an excavator onto a rock and twisted knee. Employee did not get evaluated until a couple of months after the incident date. Then had surgery in late November, resulting in this report.

Employee was at work following incident on modified duty for 9 days before EE's claim was denied and EE was sent home on personal injury leave. Employee had rested a 57"" lock ring against a tire. EE was attempting to lift the bottom half up so EE could slide it onto the top of the tire. The lock ring weighs approximately 103 lbs. As ee was lifting upward ee felt a pop in right shoulder. This task is completed on a daily basis and is routine in nature.

Miner fell through ladder opening in scaffold onto deck below. Miner sustained bruises to upper and lower back. Was transported to hospital for evaluation and released three hours later with pain prescription. Miner returned to work the next day.

An employee was installing a tensioner pulley on a D10N motor. The employee grabbed the tensioner pulley without realizing how sharp the outer edge of the pulley was. The employee was wearing latex gloves at the time of the incident.

When employee had a follow-up Doctor appointment on October 25, 2016 the Doctor reported a fractured rib making this a reportable incident.

Employees noticed smoke coming out of the cooler tower. They called on the radio to alert coworkers of the fire. Multiple employees brought fire extinguishers to the cooling tower area. Employees tried to extinguish the fire with several fire extinguishers. While fighting the fire employees were exposed to smoke inhalation. They were given a prescription inhaler.

Fall onto or against objects

3 employees were changing a bearing on a screen. The injured employee was pushing the pry bar against a drive shaft and it slipped, causing EE to fall into the drive shaft and resulting in approximately 1.5"" laceration on left cheek.

Struck by flying object

Employee went to K3 DTV to investigate a problem. The problem on the K3 DTV system lower gate was slamming hard when it closed. To fix the problem employee dialed the regulator down and gave it back to the CRO. When the gate cycled the exhaust on the regulator blew air and debris flew into employee eye.

Employee was adding a bag to the scale and when EE stepped off EE twisted EE's left ankle.

Employee was sealing a stopping with pre-mixed bucket sealant and splashed some of the sealant in EE's eyes.

Caught in, under or between a moving and a stationary object

Employee using 5 ft pry bar to position crusher toggle inside primary jaw crusher. Toggle shifted into place unexpectedly. Left index finger was pinched against crusher frame causing laceration and non-displaced fracture.

Employee was driving a clip onto master link on a chain and the hammer glanced off and hit employee's left index finger, resulting in a broken bone in the finger.

Struck by flying object

Employee struck lip protector on shovel and a piece of metal broke off and struck EE in the leg.

Rubbed or abraded

Employee was walking through final screening station when EE stopped to rub right eye. The employee removed glove and rubbed with right index finger. Shortly after that the eye became irritated and the employee flushed it with eyewash at the office. Further irritation resulted in employee seeking medical treatment.

Employee was unhooking a chain that connected a diesel scoop to a roof bolter that was hung up. The diesel scoop rolled back while employee was unhooking chain, the chain tightened and cut employee's left index finger. The lacerations required stitches.

Struck against stationary object

Employee was working under a conveyor belt, when ee stood up ee cut back on a piece of guarding, resulting in a couple of stitches.

Struck against stationary object

Employee was adjusting the roof mesh above the continuous miner when left hand slipped and struck the edge of a previously installed roof strap. Caused a laceration to the knuckle of middle finger on left hand that required 2 sutures to repair.
