The fire boss came upon the roof fall towards the end of shift ee called outside to report the fall and state and federal agencies where notified this fall occurred on the 2 west mains 16 brk. in a neutral entry beside of our track entry. No men where injured and no equipment was involved the fall. The fall is 50ft long 18ft wide 5ft thick.

An unintentional roof fall was found on 7/31/2017 at approximately 12:28am. The fall was in the 2LT intersection in the last open line, spad 5584. The approximate dimensions of the fall are 20'X18'X7'
The fall occurred at the crosscut entry between #5 and #6 intake entry 140ft inby survey station #983 located on the Southwest Mains. The fall measured approximately 30ft long by 18 to 19ft wide by 7 to 8ft thick. The fall did not impair ventilation or obstruct the primary escape way. No persons or equipment were involved. The area was blocked off and isn't intended to be cleared.
An unintentional roof fall was discovered around 6:50 AM on the mainline track haulage near the ""A"" Shaft Bottom area. The fall was approximately 16' wide by 30 - 40' long by 7' high.
Rock fall 4E belt line from break 11-14. Fall measured approximately 20' wide x 200' long x up to 6' high.
A fall of the mine roof was discovered at Brk 40 of the Johnson Fork Mains, the fall occurred from between the #4 and #5 entry over to the intersection of #6 entry. This fall measured approximately 120 ft Long X 10 ft Thick X 20 ft wide. This fall does not impair ventilation or impede travel in this area.
The #3 16ft shaft South conveyance came out of the guide in the loading pocket causing damage to the walkway and an air driven cylinder.
While a feeder was being loaded on the service hoist from the surface the crane went down in which not allow the service hoist to operate. The service hoist itself was not actually down just the crane. No one was trapped or injured and everyone was evacuated.
IGNITION - Occurred on the No.6 section in No. 1 entry as the continuous miner was cutting bottom rock and a frictional ignition occurred lasting seconds and was extinguished with a fire extinguisher
T2 Pellet Plant elevator stopped with one individual inside. Employee used gaitronics to call for assistance. Electricians were called to troubleshoot. Elevator service company was called. MSHA Hotline was called. Employee trapped insided elevator was able to open door and safely walk out of elevator after approximately 55 minutes. No injures or health effects were reported.
The main hoist at Camp Creek Portal went down due to a problem with the hydraulic brake accumulator. The hydraulic brake accumulator was repaired and the hoist was put back in service at 2:54 pm on 07/26/2017.
A roof fall occurred at/or above the anchorage point at Main North, #5 heading at 150 block. The fall does not impede passage or obstruct ventilation. It is approximately 16' wide X 8' high X 75' long. No other 50.20 criteria applies.
Four (4) men became stuck on the elevator when it stopped working.
The kiln operator received an interrupt alarm that #2 Kiln stone feeder weight conveyor belt had stopped. The Utility attempted to reset the belt at the ground level MCC VFD with no success. By the time the Utility made it up to the belt to investigate the issue it was already completely engulfed in flames.
On 7/26/17 mine management was notified of a fall of ground at 2300 East 2000 A5 stope that went beyond the deepest support at 4:10 pm. Mine management called the MSHA hotline at 4:25 pm and was issued a verbal 103K order from MSHA at 1705 hours.
Employee working at the north block water fill stand noticed brush fire flames in the area 200ft adjacent to EE's location. No one else was working in the area. EE notified the supervisor. Fire rescue and water trucks were dispatched to the area. The ground area burned was approximately 1200ft by 600ft. The power line poles in the area did not receive any substantial damage.
An unintentional roof fall occurred along the #4 belt at break #26 one break inby spad #1419 between the #6 and #7 entry in the primary escape way approximately 9,900 feet from the active face. The area was dangered off and the life line was rerouted. This area will not be rehabilitated. The fall was 40 feet long X 20 feet wide and 7 feet thick.
There were broken strands on the Crabapple Hoist rope found during routine inspection. The hoist was immediately taken out of service.
Employees in the Service Shaft were cutting out an old 6"" diameter air line when a piece of hot slag started a small methane fire in the shaft approximately at the 689.5 level. Methane was coming out of a joint in the concrete in the shaft. When the employees discovered the fire, they went to the level of the ignition and extinguished the fire with a fire extinguisher.
PC490 backhoe & D-11 CAT were draining trapped water on north side of surface dump. D-11 was pushing the ditch material away from trench over dump edge when berm began to fall away & trees in front toppled over. Both operators left area immediately. The dump edge sloughed along with the trapped water.
While inspecting the hoist and hoist rope at RV1 portal, the inspector found that the service hoist thimble connecting the service hoist counter weight rope to the counter weight had slipped through the thimble and was not secure. The hoist was taken out of service immediately and was reported to the 800 number.
Unplanned roof fall in the #2 heading at #9 block on the 12 Right Longwall section. Approximately 16' wide x 16' long x unknown height. Also projected into crosscut 2-1 for a short distance. No injuries or equipment damaged. Dangered and timbered off.
A non injury roof fall was discovered in the #3 belt line entry at cross cut #75 in the 1 west mains #4 belt line. The fall appears to be approx. 25 ft wide, 20 feet long, and 8 to 10 feet thick and is in an intersection. Neither ventilations or escape ways are effected. Area has been dangered off at all points of entry.
The 20 person elevator was not operational due to brake issues, repairs were made and mine resumed work at 10:15AM
A roof fall in the intake escapeway was discovered at 2:15 PM at 37 X-cut, #3 entry in the Cedar Grove Mains Intake.
Lightning started a brush fire at Collom. The fire burned for over a half hour so it was reported to MSHA. The fire was discovered at 2:50 pm and was extinguished by a rain storm at 3:39 pm. The fire was in a remote location and no structures, equipment or personnel were threatened by the fire.
An unplanned roof fall was discovered in the #3 Entry at crosscut 13-09. The fall is approximately 20' wide 20' long and 10' high. All accesses have been posted and dangered off.
A fall of roof occurred in the 4th SE Mains at 58 xc, entry # 4. The fall measured approximately 18 feet wide x 30 feet long and the thickness is unknown.
Lose power on the camp creek portal side of Shoal Creek mine. The power lose effected the #7, #8, bath house and camp creek hoist. The hoist was in operation at the time with one person on the hoist. Power was restored and the person was removed from the hoist with no injuries at 8:19 am.
There was one employee trapped on Camp Run Elevator for approximately 38 minutes. The cause of this incident was found to be a ripper fault on the elevator.
Four men were reported as being trapped on the Fish Creek elevator at 12:42 pm. The men were inspecting the elevator when it traveled below the safety switch causing the elevator to be unable to return to the surface. All four men were of the elevator and standing in the Intake until the elevator was returned to service and at that time traveled to the surface.
This was a planned impoundment maintenance procedure to remove and burn woody plants. When smoke appeared after the burn, a trench was excavated to the depth of the coal fines to prevent lateral movement of the burn and resident clay material was placed on top of the smoke.
While traveling on the vertical elevator to the mine bottom, the elevator stopped. The employee remained in the elevator (in excess of 30 minutes) until the part was replaced.
Roof fall was discovered on 07/18/2017 in the #6 Entry near spad 13344. Breaker posts were installed and all approaches to the fall were dangered off.
A roof fall above anchorage was found by the weekly examiner. This fall was due to normal weathering. The fall was 18x60x6 and blocked the travelway of the examiner. There were no injuries with this accident. The ventilation was not interrupted and the area has been dangered off at all approaches. This area was mined in Nov. 2014.
Fire started in the median on Interstate 80, presumably from a cigarette thrown out of the window from a commuter. The fire crossed eastbound lanes and spread onto the mine site where it burned for approximately one hour before crews extinguished the flames. Employees were evacuated as a precaution. No injuries occurred.
At 7:45am Outby Foreman discovered a roof fall in #7 Entry West Mains #1 Section. The fall fell above anchorage point and measured 110'x19.5'x8' thick. Cribs/timbers were built in #6, #7 and #8 entries and stopping between #7 and #8 entries. This area was mined mid March 2017.6' glue bolts and 8 - 10' cable bolts were utilized.
A grass fire was discovered burning in a remote area next to a substation. The fire was burning away from the substation and any active work areas. No miners were endangered and emergency personnel responded and put the fire out. Extremely dry conditions in the area has the fire danger very high.
Roof fall was discovered in the track entry of the 6 North Haulage at 17 block. The roof fall was an estimated 16' in width x 28' long, and fell 9' above the coal seam. The area was initially bolt with a two piece combination bolt. No injuries resulted for this accident and ventilation was not impeded. A 7 man personnel carrier was trapped under the fall.
The roof fall was found by our fire boss on #18 belt in #5 entry, #3 x-cut, in the secondary escapeway. The measurements of fall were as follows: 20' in length and 35' in width and the cavity is approx. 14' high after fall. The area was bolted with 5' fully grouted bolts. There were pie pans, bacon straps, and banana plates in this area.
Portion of the side wall (rib) had fallen due to water erosion by a nearby sump impairing two stoppings at the bottom of shaft #3. In order to effect repairs in this area, power and ventilation were shut down after mine employees were evacuated. Repairs were effective, power and ventilation were restored within two hours.
A non-injury roof fall approximately 19 feet wide, 70 feet long, and between 7 - 8 feet in height occurred between breaks 13 & 14 on the 8B4 belt line.
As a loaded train was leaving the mine a grass fire was started on the rail loop, fire pushed north onto private property. Fire was started on Buckskin rail loop so fire was called in on 7/13/2017 to MSHA. No hurts or equipment/structure damage. Campbell County Fire department extinguished the fire.
HAD A ROCK FALL OUTBY # 1 UNIT IN # 9 ENTRY AT 9+00 TO 9+50 IN PILLAR LINE. Roof Fall was about 18' wide x 20' long x6.5 high
An unplanned roof fall was discovered at 4:35 pm at 16-17 intersection on the belt line. The fall was approximately 15'high 16' wide and 80' long. The fall was cleaned, bolted and the area was restored to safe passage.
This area is part of the 7 day exam found by the examiner on their normal run. A roof fall was found which was 18x20x6. Travel way was blocked due to the fall, ventilation was not affected. The fall is due to normal weathering. This area was mined in March 2006. There were no injuries with this accident. This fall was located in the West B right side return at spad 37+64 H entry.
In the process of transporting men when elevator stopped working.
MSHA inspector discovered a small fire in the berm beside a coal stockpile. The high sulfur coal in the berm has spontaneously ignited due to an increase in the ambient temperature. Burning material was spread, watered, and compacted to extinguish fire. The adjoining stockpile was watered and spread out to eliminate some build up of heat.
A production drill was drilling over an unseen void when the drill fell partially into the void. As the extent of the void was unknown at the time the operator remained in the cab until exit could be assisted by mine rescue personnel.
At 7:05 pm on July 10, 2017 a gas ignition occurred on the H-3 section in the #3 entry while the continuous miner was mining.
A fall measuring 20' wide X 25' long X 4.5' thick occurred at 140' inby survey station 925 in the #7 right return entry on the Northwest Mains. The fall will not be cleaned up. We are re-routing the return around the fall. No persons of equipment were effected. All entrances to the fall will be barricaded with timbers or cribbed off
Mine examiner discovered a roof fall during the course of examining the mine. Felled material was laying against the refuge chamber.
Unplanned roof fall on Empty Track near rotary dump. Approximate dimensions are 16'wide x 7' high x 30' long. No injuries. Fall was discovered during a pre-shift examination.
Smoke and flames were observed coming from the exhaust of a haul truck. The engine continued running after the ignition was turned off. All personnel were withdrawn from the mine as smoke filled the return escape way. It was determined that the engine turbocharger seals had failed allowing lube oil into the engine intake resulting in the smoking exhaust.
The guide rope for the escape hoist located at cross cut 89 on west main was found damaged during a weekly inspection of the hoist. The damaged guide rope was repaired and the hoist was put back in service at 11:40 am on 07/07/2017.
#3 hoist tripped out and would not move from the collar position. Cycling power to the drive was needed in order to get the hoist to move. The drive was returned to all original parts following the changes that had been made the night prior while troubleshooting. The hoist operated normally following the changes.
Pleasant Grove Elevator was down due to a faulty door switch.
Had a rock fall in the 3rd. South Panel at 8+50 in the intersection. The fall was 19' wide x 19' long x 9' high.
IGNITION - Occurred in the No. 1 entry on the No. 7 section as the continuous miner was cutting coal and a frictional ignition occurred lasting seconds and was extinguished with a fire extinguisher.
Employee was preparing to send supplies underground via the #3 hoist. As soon as the operator moved the hoist's joystick to send the cage underground the hoist tripped out. Shortly after this MSHA was notified. After several hours of troubleshooting it was found that there was a broken lug on the DC loop breaker.
Found an unintentional roof fall on number 3 belt entry in Combs mains between breaks 26 & 27
The service hoist at the Camp Creek portal went down at 4:20 pm due to a power outage caused by an electrical storm in the area. After examination of the hoist it was determined that the hoist would not operate due to a bad rope slip switch. The rope slip switch was replaced and the hoist was put back in operation at 8:14 pm.
An unplanned roof fall was discovered at 6:35 pm at 16-24 intersection on the belt line. The fall was approximately 12' high 16' wide and 30' long. No injuries occurred. The fall was cleaned, bolted and the area was restored to safe passage.
IGNITION - Occurred in the No. 2 face of the No. 7 section. The continuous miner CM128 was cutting bottom in a forward direction when a frictional ignition occurred. The ignition lasted only seconds and was extinguished with fire extinguisher.
During daily inspection, a malfunction was discovered in the North Elevator. Originally thought to be an electric motor failure. The actual failure was in an electrical relay. The elevator was up and running around noon on 07/03/2017.
Hot material from the cooler fell onto cooler product belt and burnt the cooler product belt up.
Employee in flatbed service truck was parked on a waste dump at the Mission Mine. A 320 ton haul truck ran over flatbed service truck resulting in fatal accident.
A Cat D9L bulldozer ee had been operating ran over ee. The bulldozer ran over the edge of the refuse bank and came to rest at the bottom of the embankment. The victim was pronounce dead at the scene. The reason why the victim was outside the bulldozer when the accident occurred is under investigation.
Cracked stone fell and crushed employee. Employee didn't follow procedure. Employee's lanyard wasn't tied off and the employee was using a foot wedge and jackhammer with 1 1/4"" bit. Employee should have been using 3/4"" plug drill and 5"" tether and wedge. The employee was drilling from the base and should have been drilling at the top.
Temporary contracted employee contacted the 12500 volt overhead power line with an irrigation pipe
EE was installing rib bolt when drill steel came out of chuck. EE reached to position steel & hit the rotation lever instead of the feed lever. EE caught a finger between steel and spinning drill head. The skin at tip of finger is torn off. EE had surgery on finger & they removed the bone at tip of finger to repair skin around end of finger. EE is off work & will have lost time.
Temp employee removed a safeguard to clean out a clogged valve. EE proceeded to clean out the valve with finger. EE did not shut off or lock out the machine. EE right index finger was cut off at the first knuckle.
A front-end loader traveling on a mine road broke through into a water conveyance ditch and overturned. A contractor noticed the overturned equipment and responded to the site. Contractor employees immediately notified Mosaic supervision who activated the site ERT. ERT contacted 911.
Demoing brick from an autoclave with a titanium baffle. Titanium baffle was supported from side to side movement. The employee reached to brush rubble from under the baffle and the vibration from use of the rivet buster caused the baffle to shift, pinching the right middle fingertip of the employee.
A miner was injured while in the process of changing the canopy post on a twin boom Roof Bolter in MMU 006-0. As the canopy was being lifted off the operators' side, the canopy fell and caused pinched tip of the injured miners' finger.
Employee was pulling excess hi-line into break slipped on loop of cable covered with mud causing employee to strain lower back.
Employee was walking to the roof bolter and twisted right ankle.
EE was unloading roof bolt plates and roof bolts from the cage when they felt a sharp pain in their hand and swelling about 30 minutes later.
After an employee picked up a bag of concrete and turned to walk, their knee ""gave out"" causing pain.
Injured employee was removing a vent tube on the 13 Right section when a piece of rib rolled off and struck employee's lower left leg and foot resulting in a bruise.
While removing and installing screens at the crusher worker felt pain in left shoulder.
Employees was walking in the right recovery chute in E28 longwall when employee slipped on a piece of belt in the walkway. Employee braced for the fall with employee's right arm and felt a pain in employee's right shoulder when employee landed. MRI results pending.
The employee was bolting on the intake side of the continuous miner, while retrieving a roof strap from the strap rack on the miner EE got EE's left hand caught between an upright standard on the strap rack and an upright standard on the bolt rack causing a laceration/avulsion to the back of EE's hand. The injury required 12 sutures to repair.
Employee was walking from door to end of bare beams and tripped & Fell into set of cribs and bare beams causing bruising to hip, ribs and elbow.
EE was operating a loaded shuttle car when EE ran thru a rut in the roadway causing EE to jar EE's back. EE was diagnosed with a lumbar strain and is receiving PT. This is being reported as a NDL accident.
Employee was standing in the last open cross cut between #3 & #4 entry's, when a piece of top coal fell out between pins striking them on the left shoulder and forearm. Employee started missing work 8/31/2018.
Employee stated was carrying a bag of rock dust to the face area when foot got stuck in the mud. As employee pulled foot out of the mud felt pain in left knee, hip, and ankle. This resulted in a strain and lost work days.
Pulling hose off fuel truck, felt sharp pain above navel . First day off work is 8/4/17
Employee stated that EE was attempting to stand up after placing crib blocks under the section feeder and felt a pain in the left knee.
Drilling pin hole with jackleg drill, pulled drill steel out of hole and smashed finger on rib - fractured middle right finger between drill and rib. ER did reduction on July 28th. Surgery on August 1st.
Employee leaned beyond work zone while standing on ladder. Foot slipped off of wrung and employee fell striking the second lung on ladder.
Employee operating diesel locomotive pushing supply cars into track switch when locomotive overheated and shut down. Supply cars pushed locomotive down track. Employee trying to stop locomotive and cars, fell off injuring left knee. Employee did not violate any rules or regulations and has a good attitude for safety. Employee has proper training and was wearing required PPE.
EMPLOYEE WAS STACKING BAGS ON PALLET. EE PLACED A BAG, TURNED TO RETURN TO THE BAGGER AND CAUGHT FOOT ON THE CORNER OF THE PALLET TWISTING RIGHT KNEE.
Employee was attempting to change a belt cradle at 8-21xc when they pinched their small right finger between the roller and cradle. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was rolling track rails with a metal slate bar and the bar kicked back striking ee in the R cheek area causing a laceration requiring stitches.
Employee was pulling bolter cable and felt a dull pain in lower right back.
Employee was pulling an old belt pin out of a splice when a piece of the pin or the vise-grips hit employee in the eye. Employee was wearing PPE.
A fully trained miner injured lower back when shoveling belt. The injury was not a result of any compliance issues nor were any protective guards involved.
A fully trained miner injured employee's left leg when employee was moving a rock on the continuous miner. The rock shifted the wrong way and contacted employee's left leg causing a contusion. The accident was not a result of any compliance issues nor were any safety guards involved.
Employee received a first degree burn to right cheek and chemical burn to eyes from kiln feed during an inspection. EE was not wearing proper PPE. EE was treated at the hospital and then later by an eye Dr and by another Dr. ee will be released to go back to work the evening of 8/7/17.
Angle iron removed during maintenance, stored on end, fell over, striking employee.
The above shows original date of injury. The employees first missed work day was 8/2/2017. Description: The employee was climbing in and out of the miner conveyor chain making repairs. Employee felt a pop in employee's knee.
IE said while hanging miner cable with a co-worker, the tie wire broke so IE reached to catch the cable with left hand. IE continued to work but noticed that hand/arm felt different. Note: This employee had a previous injury to the left hand/arm (tear to distal bicep) that required surgery to repair. Surgical repair was performed again following this re-injury.
Employee was installing a post when they felt a pain in the elbow. Finished the shift and visited the doctor the next day. Worked restricted duty until 08-23-2017 when they called in saying the doctor was taking them off work completely.
The operator of 23003 haul truck reported to the supervisor that they had bumped their head on the radio while tramming back to get loaded.
EE removing jumbo box off bolt, S hook failed crushing the EE's right hand between the bolt and the chain. EE scheduled for surgery on the right ring finger on 7/27/17. EE off work until post-op appointment.
Employee was riding in manbus that was struck by a motor causing injury to hip and lower back. EMPLOYEE HAS BEEN WORKING SINCE THE DATE OF THE ACCIDENT AND DID NOT GO OFF WORK UNTIL 12/28/17 DUE TO SURGERY ON LOWER BACK.
While troubleshooting the wiring for the rear lights on a Caterpillar 992 loader the employee slipped and fail when crossing from one side to the other.
EE was walking bent over (~4' coal) to the section. When ee turned head to signal oncoming equipment ee felt a strain in neck. EE developed a headache shortly thereafter. EE was taken to the hospital by ambulance.
Changing out AC Compressor. Right hand slipped and made contact with fan blade causing laceration to right index and middle fingers.
Employee experienced pain in lower back and right leg near the end of previous shift.
Employee was attempting to put the forks back on a Caterpillar 930H loader that had fallen off while picking up 6' belt structure. While trying to slide the bottom of the fork into place the top came out of the groove and overturned, striking the employee on left leg, fracturing the tibia and fibula.
Miner was changing rotation on a motor. Did not properly Lock and Tag out the equipment. An arc flash occurred and the miner received 1st degree burns to hands and forearms.
It was a sunny day and 85. Employee was attempting to break the oil filter free and fell off the cone crusher.
Employee unhooked safety harness and started to descend a ladder secured to a portable crushing plant when their foot became entangled in the ladder about half way down (approx. 6' off the ground). They pushed off the ladder to try and avoid injury, but fell hard on their right side, fracturing 7 ribs, and collapsing a lung.
Driller was changing steels and smashed finger between the 48"" wrench and drill mast.
Employee received UV burn to eyes from looking at another employee welding
Employee was walking through an open air lock door when the door closed back on employee's left foot. Employee was taken to the hospital to be checked. Foot was only bruised and employee was taken off work for two days. Employee is now back to work.
Employee was changing bits in E29 Longwall on the tailgate drum when a rock fell from face and landed on left foot.
Loader operator was mucking out a shot when a rock fell from the roof and struck the top of the loader's ROPS, jostling the operator. EE suffered a broken rib and was hospitalized. There were no life threatening injuries.
Employee reported that while wearing safety glasses, ee got particle of foreign material between eye and contact lens.
Cleaning alumina inside clay bailey opening on screw conveyer housing - Screw caught two fingers creating open fractures
Employee had just finished scaling top and had returned to the drill station to start bolting, as EE reached for a bolt a piece of rock 14""x19""x1 1/2"" fell and hit the top of EE's right hand. EE was taken to hospital, x-rays were negative. EE had to wait to be seen by orthopedic and the orthopedic has EE off for 2 weeks for physical therapy.
The employee stood up from repairing maintenance work on a shuttle car. When employee stood up, employee struck employee's head against the mine roof. This caused employee to stumble, lose employee's footing, and then sprain employee's left ankle.
EE climbed up on ladder of Komatsu 785 Haul Truck to inspect and secure a block heater receptacle that was broke off. When climbing back down off of the truck EE strained lower back. EE finished the shift and did not report injury till 1:00 pm on 7/25/2017.
Was trying to dislodge a rock from the jaw. Something popped in employee's back.
Employee was operating a 785C Rock Truck along the haulage road to the valley fill area and lost control of the truck and went through the berm. The truck overturned and came to a rest at the bottom of the valley fill road. This resulted in fractures to the lumbar vertebrae and lost work days.
Employee was walking to the loader in the 8 South Section 7-8xc. Employee stepped in a crosscut for a shuttle car to come by when a corner fell out striking employee in the legs resulting in contusions to both legs. Miner proficiency, skills or attitude was not an issue. PPE was not an issue.
Employee was in the process of placing a drill steel in the drill mast. EE had one end on the pick-up tail gate and the other end on top of a berm. When employee pulled the steel onto the mast the back end of the steel fell off the tail gate pinching the employee's left hand between the steel and a large rock laying on the the berm.
Employee was loading an ATRS jack for a continuous miner onto a mantrip to transport to a section. While maneuvering the part it shifted an fell pulling employee's right shoulder downward. Employee did not violate any laws, rules or policies, is properly trained, and wearing proper PPE. Has a good attitude for safety.
Employee was walking across shot rock to the foreman's truck when their right foot slipped off of a rock and into a crevice. This caused the foot to bend backwards. Went to ER that night to get checked due to increasing pain. X-rays were negative but lost time started on 07/25/17.
Miner felt pain in left foot and ankle after prolonged standing on ladder.
Employee was mining in #7 entry when a piece of draw rock fell and hit ee on the head. Employees left knee had soreness so the doctor had ee off for 2 days. EE returned to regular job 2 days later.
Crew were loading I-beams across the belt line. EE attempted to lift the beam by self to place on the belt line.
Ee was removing hydraulic cylinder, the cylinder slipped, finger crushed. Seen at ER and diagnosed with a crush injury and referred to an orthopedist. Orthopedist confirmed fracture and released ee with restrictions that site cannot accommodate.
EE stumbled & fell when ee got off mantrip. EE tried to catch self, felt sharp pain in left wrist, wrist is swollen. EE went to doctor & was given FULL DUTY RETURN TO WORK RELEASE. EE followed up with doctor a week later & was taken off work on 08/08/2017 until evaluated by specialist. EE was diagnosed with a ligament tear in left wrist.
The employee was loading roof bolt glue onto a scoop when they felt pain in the left shoulder.
Batteries ran down (no power) on the mantrip vehicle used to transport 2 people (spad crew) to the surface. One of the occupants head/cap got wedged between the mine roof (30' coal hgt.) & the vehicle frame & when machine movement occurred slightly, the occupant received injuries. Diagnosed as concussion and neck muscle strain.
Miner was helping to change the attachment on the excavator from a hammer to a bucket. As the miner tried to loosen the nut with a wrench, EE propped foot on the attachment for leverage. When the nut broke loose, the miner fell to the ground, landing on EE's elbow. The miner suffered a dislocated elbow.
Employee stated that while EE was lifting a 20 ft X 6 inch Victaulic water line up onto a mantrip, EE felt something pull in the left shoulder. EE and co-worker were the only two that were there lifting the material and picking up trash.
EE was prying rollers off stack, slipped in gravel and struck skid on lower leg under knee. EE worked till September 12, 2017 when ee went off for surgery to repair knee.
Employee turned in a accident report on 7-20-2017 stating struck their head on mine top or key hole plate causing pain to neck. No lost days or restricted duty until 9-8-2017 employee went back to Doctor complaining with pain to upper back and neck is currently off work pending test.
The employee was recovering high voltage cable when the cable fell from the mine roof it struck the employee on the L wrist. The weight of the cable caused the employee's wrist to bend back fracturing L wrist.
Mine Foreman received severe electrical shock while welding on primary impactor. EE received burns on chest and mouth.
Completed project, carrying survey equipment slipped and fell
A fully trained miner was injured when the miner's rail mounted man trip was struck by another rail mounted man trip.
Employee knelt down to show a fellow employee something on the scissors lift. Employee then stood up and felt severe pain and described the situation as ""my back went out"". Employee was released from work and went to visit a chiropractor.
Employee was on knees bolting, when ee twisted and felt left knee pop.
Employee was checking under the hood of the EDV company number 2783 with the engine turned off when the radiator hose busted spraying the employee on right side of body.
IE was adjusting the forks on a minitrac. IE lifted on the fork to adjust it. Lifting on the fork rotates the keeper stock located on top of the fork. As the IE lifted the fork they did not realize that their little finger was in a pinch point between the metal stock keeper and the frame. IE received 9 sutures.
Employee was coming down out of the haul truck when EE fell on EE's behind and had pain in the back.
Employee was packing two buckets of miner bits across the section to the miner and employee's back gave out causing employee to fall to the ground.
Employee was working at the back MBC when foot was stuck in the mud. As employee turned to walk away felt something pop in right knee. Employee started missing 07/24/17.
EE was working on #9 Tertiary Dust collector when ee felt something crawling inside shirt sleeve. It bit ee. EE squashed the bug & it rolled out of sleeve. EE went home w/o reporting. The next morning ee woke up to find that arm was swollen & the bite site had become infected. EE had pain in arm. EE sent to clinic, taken off work & given antibiotics for infection.
Employee was walking down the catwalk on stacker belt and went to step down the step when their foot flew out from under them.
EE was walking underground and stepped on uneven mine floor twisting right knee. EE worked normal occupation without restrictions until surgery date on 9-5-17. EE will begin missing time 9-6-17.
On 7/18/17, EE was setting up a crane with the crew. While pulling the cable thru the block of the crane EE injured EE's shoulder. EE sought medical treatment & was diagnosed with a strain. EE recently went back to the MD & was diagnosed with a rotator cuff tear. EE has continued to work regular job. EE will be having surgery in Nov.
On 7/24/17 employee reported to the supervisor that on 7/18/17 at the end of their night shift. They injured their groin area when the bolt cutters they were using slipped out of their left hand contacting their groin. Seen by site medic referred to ER. Restrictions were issued.
Contractor was said to have been struck in the back by a rock of unknown dimensions while servicing a miner. EE began missing work the following shift.
Was building a wall when grabbing another block from pallet when top block slid off striking hand
Employee did not report a new injury at this time. Employee requested information on a previous injury from 2012. Stated the condition from the previous injury had returned, Old insurance company reported that the employee stated they picked up a bag and felt something in the same location of the previous injury.
Employee was attaching a tie to the track using the bucket of a scoop to lift the rail. When the scoop operator raised the rail with the bucket. The rail slipped out of the scoop bucket and fell on foot.
At 1100 AM while employee was checking screen inventory employee injured employee's left forearm and left knee. The first lost workday was 8/23/2017. The shift started at 600 AM
The employee was walking beside the Rail Road cars at the load out facility checking the doors on the cars. The employee stepped onto a rock and the rock rolled over and the employee fell into a small ditch. The employee continued to work and did not receive any medical treatment until 7/19/2017.
Driver exiting and only caught bottom step with their toes. Their foot slipped off and sprained ankle when they hit the ground.
Employee was driving a haul truck down a haul road and rolled the truck over.
Injured was attempting to hang miner cable slack (between miner and loader) in 1 left to 2 left at 5 block of 1EHG longwall setup development section. A piece of draw rock slid off a roof strap (2' diameter, 1/4-1"" thick) struck injured's hard hat, went down their body resulting in minor contusions and abrasions, most severe injury was a sprained ankle.
Employee slipped on a piece of slate twisting right knee.
After a day of welding on Monday, July 17, 2017, the employee awoke at approximately 2:00 am the following morning with a burning sensation in their eyes. The employee reported to work on Tuesday, July 18, 2018 but left to see an eye specialist once they were open. The doctor diagnosed the employee with welders flash and prescribed eye drops.
Employee had finished drilling a round with an electric/hydraulic jumbo, after turning off the electrical power employee was lowering the power cable that was hung along the rib and felt a pain in right shoulder, pain continued to be persistent. Upon reporting seen by site medic and referred to ER where restrictions were issued for a strain to rotator cuff tendon
Employee was using a sledge hammer to unplug chute when ee felt a pop in left shoulder. Employee went to doctor on 7/20/17 for MRI. On 8/2/17 it was determined that ee had a torn muscle and that it was work related caused by the incident. The employee is currently off work.
Employee was holding pry bar with left hand and was hitting it with 8 lb hammer when they missed the bar & struck their left hand. EE finished the shift but went to physician on 07/16/2017, next day. EE was diagnosed with a bruised radial nerve. EE was taken off work for 3 days and returned to work on 07/20/2017.
Employee was unplugging de-energized charger plug from #69 cat scoop in the 4E section. When the scoop plug arced causing burns to the employee's abdomen. Cause of accident was improperly wired breaker and loose plugs lead. Miner proficiency, skills or attitude was not an issue. PPE was not an issue.
Employee was bolting in 3 heading when glove got hung up on the bolt and twisted arm around the bolt.
The injured employee was assisting with dropping high voltage cable from hangers to check plugs. While dropping the cable, the injured employee was struck in the head, shoulder and back with the high voltage cable. The employee continued to work until April 5th 2018, at which time, a physician placed them off from work until May 4th 2018.
IP was in process of completing a delivery to customer site. Foot became tangled in rope between wheel chocks causing IP to fall. IP place arms out to break fall and when landed caused fracture of the bone.
Employee was installing rib bolts and slipped and fell off bolter platform and hit back on drill mast. Bruised back.
Employee stepped out of sweeper truck, when their right foot made contact with ground their ankle turned over, twisting it. They returned to work on their next scheduled shift and started missing work on Monday July 24.
On 7/17/17 the employee requested medical treatment, was diagnosed with a right hip strain and was placed on restricted duty which is being accommodated. The employee was laying on their back on top of a bottom deck screen pushing out one screen panel toward the discharge end with both legs in a squat type motion resulting in R-Hip pain.
The miner was assisting a welder with fabrication activities when slag entered glove cuff and burned three fingers. The miner declined treatment at the time. The following Monday (7-17-2017), the miner complained of pain and swelling and was sent to an urgent care for treatment. Did not become a lost time accident until 7-18-2017.
Employee was stopped on haul road in a Cat 777 haul truck waiting on an approaching Cat 785 haul truck. When the approaching 785 truck was blinded by the sun while leaving the coal yard and struck the parked Cat 777.
Employee was dismounting a 988F Loader when they slipped, as the employee caught themselves they felt a pain in their right shoulder.
Employee was removing bolt from water pipeline discharge flange with a 15"" crescent. Discharge hose was attached to the pump which was suspended about 4-5 feet. While applying pressure to loosen bolt/nut the wrench unexpectedly slipped off of the nut resulting in handle hitting the employee in the forehead.
Employee was troubleshooting a flow problem on the belt press. When EE pulled a hose off, the fluid sprayed EE in the face. Employee states EE knocked safety glasses off when the fluid startled EE.
Laborer slipped when descending from conducting drop box maintenance on a 1 1/2 in stacker. Suffered a bad ankle sprain from landing on the ground. No one witnessed the incident. Laborer reported missing foot hold to supervisor, but told doctor at clinic employee slipped from a ladder.
Employee was using a wrench to hold the head of a bolt while tightening with impact. Felt a pop in employee's left bicep area. Taken to Doctor same day, returned to work without restrictions. Returned to primary doctor, returned to work with restrictions on 7/28/17.
Worker was shoveling feed to clean area. Employee's feet slipped on wet and oily surface and worker fell back onto an approx. 8"" pipe with a 12"" flange causing pain to lower back.
Employee was rock dusting header area when EE stepped on unlevel grade causing EE to trip and fall spraining right ankle.
EE was helping remove a Cement Ring out of a finish mill. EE was laying on EE's left side with EE's knee in cement dust. EE received minor chemical burns to the left side of EE's neck, creases of both elbows & EE's left knee. After finishing EE's work, EE washed off and continued with EE's day.
IE was hanging high voltage cable. The cable got hung on itself coming out of the sled causing tension on the cable. This energy pushed IE into the sled twisting right ankle. Employee had scheduled vacation starting the next day. After returning from vacation and still experiencing pain, IE returned to doctor and was taken off work 7/24/17.
Miner fell from tire of Cat Motor Grader, landed on back, fractured rib. Miner reported accident, but did not report injury until 07/24/2017.
Injury occurred when it was night time. Employee was working on jaw crusher and went to mechanic truck to get a bolt and nut. Employee tripped and used arm to catch the fall and ended up straining right shoulder.
While building a wall the employee lifted a cement block and twisted feeling pain in lower back. The employee reported to us 7/31/17 that the employee had Doctors excuse that the employee was unable to work. This was the first time the employee notified us of the injury.
Employee lost control of truck while descending a ramp and rolled over.
The employee was tightening flange bolts on an intake line connecting to a water pump. The employee said that they felt a sharp pain in their right shoulder while tightening the nuts with 2 crescent wrenches. The employee saw a physician and was given 2 medications for the soreness and swelling. The employee was released to come back to work on 07/17/2017.
Holding plug on jumper cable while two others hit it with hammers to separate the jumper cable plugs (3E longwall). Metal shard came off hammer and hit EE in abdomen. Persistent discomfort caused EE to seek medical attention, which discovered a shard of metal embedded in EE's abdomen. Started missing work 8/1/17 to have the shard surgically removed.
The employee was walking outside of the mill 18 area by the tramway of the ball mills/NTR area. There was a defined clean walkway, but it appears that the employee walked through a buildup of material that was not on the walkway. Twisted right knee.
Employee was on knees shoveling beltline and removing belt structure that was stuck in muddy bottom conditions. Employee felt pop sensation and pain in lower back when trying to remove stuck piece of belt structure.
The employee was using a tool to exert force to loosen a nut from the air compresor, slip and hurt their back.
Employee was taking D11R Dozer up on shot when the face of the shot broke causing the dozer to turn over hurting right thumb and spraining left ankle.
Employee slipped on slick hardened salt causing an awkward fall on employee's ankle. Employee went the Dr on next scheduled shift (7/12/17) The doctor diagnosed employee with a sprained ankle and employee could not return to work. This became a lost time.
On 7/11/17 EE reports that at approximately 10:30am EE fell to right knee and hit a rock while carrying a grease barrel to the dragline.
Employee was letting some rags tied to a rope down from a catwalk under u-34 belt and caught left arm in between the bottom return roll and the belt.
Employee did not report incident to the supervisor until 7/13/17. Employee was servicing a truck when EE's arm brushed a portion underneath the truck knocking dirt loose. Some got into EE's right eye.
A Non-Mine worker assigned to perform remote cleaning of conveyor system counterweights was left unattended by Qualified Miner. Non-Miner identified smoke coming from a damaged electrical line on a floating pontoon from the operating dredge. Non-Miner & 2nd Non-Miner went to investigate. While standing in water, at edge of dredge pond, worker contacted metal pipe.
Employee was getting belt pins off the dolly when a rock fell and struck them on the back of the head and knocked their face against the lolo dolly.
A contractor employee was loading the roof bolter and a piece of loose material fell from on ee right foot. EE was not given a full release due to fear of infection of a wound on right big toe.
Employee observed a large rock going through the feeder that was going to contact the ""dead-man"" cable going across the feeder. EE attempted to hold the lever that is pulled into the e-stop to avoid the feeder from stopping, but the rock hit the cable & pulled the lever mashing EE's finger between the lever & the e-stop causing it to fracture the end of EE's little ""pinky"" finger.
While roof bolting, miner complained of left knee pain and stated heard a ""popping"" sound. Miner was transported to local hospital, treated and released. On 08/22/2017 miner underwent surgery of unknown diagnosis therefore became a lost time accident on that date.
Employee was putting tire on shuttle car and was lining key steel up on hub when employee strained or over-exerted self some how as employee was pushing, turning, and lining it up. Employee started missing 07/13/2017.
EE was cleaning the Mill MCC Room when EE tripped and fell, landing on right shoulder. EE refused medical treatment, reporting was sore but OK. EE requested examination on 8/14. EE was examined on 8/14, 9/6 & 9/12/17. Soft tissue surgery recommended after 9/12 exam. Surgery occurred 10/18/17.
Miner fell of a distance of 11 feet from a conveyor system onto steel deck.
Miner in process of repairing roof bolter dust skirt out by face area when draw rock fell from roof striking left leg which was positioned on mine floor. Miner sustained an inline fracture of left tibia and ankle.
EE was walking and stepped on a rock and heard a pop in right knee. Pain was minimal EE did not seek medical attention until 7/19/17. EE treated as FA and released back to work with no restrictions or pain meds. EE went back to Dr on 8/24/17 and placed on modified duty. EE stated knee still pops and pain is still lingering.
Employee slipped on mine track and fell twisting back and feeling a sharp pain in mid to lower back.
Employee ascended the stairs on a Komatsu500 loader. Employee was cleaning the loader. Employee PPE:boots,hi-vis,earplugs,hardhat,goggles,textured nitrile gloves. Task required a different implement. Employee said their goggles fogged when they reached for the handrails to descend,causing them to miss the rails and fall,resulting in broken vertebra,L5 and T12 rupture.
Employee was shoveling belt line on section, employee stepped on loose rock & twisted left knee. Employee's initial treatment was at the ER & was instructed to follow up with employee's physician. Employee has loss time, first day missed was 07/09/2017 and is still off.
EE was helping pick up the stacker. (ST-02). When hooking the chain up, EE felt a pull in the right shoulder. This EE had not received any work restriction that affected the job at the mine site, but did have restrictions. EE was released as of 9/5/2017. Disputed EE's claim & now moved forward with shoulder surgery. Completed on 4/19/18. No lost time or modified duty till surgery.
When stepping down from a conveyor, the miner miss stepped and fell striking against another conveyor.
Employee was at the end of bolting cycle when a rock contacted on the left side of the hard hat. EE was seen by the site medic and referred to the ER, X-rays were negative, restrictions were issued for a cervical strain.
Operator noticed a hydraulic leak on track hoe. Got out of equipment to see about leak and clean up the oil when ee slipped on track hoe and fell hitting right side of body on track hoe and landing on pile of rocks.
EE was walking to the CM when ee twisted knee as ee was stepping over the coal pile behind the CM.
Employee was taking slack out of the face chain, went to reach for a dog bone ear to put in thats when their foot slipped in between face chain. When they tried to pull foot loose they felt a pop in left knee. EE worked till 8/08/2017 when they went off for surgery.
EE & another employee were replacing top roller at mainline #4 belt drive. EE bent over attempting to lift roller up on belt rail when EE strained EE's back. EE went to local hospital and was diagnosed with pulled muscle.
A fully trained miner was cutting overcast when something came into contact with their left eye. Miner did not seek medical treatment until 7/24/2017 and returned to work 7/26/2017. The incident was not the result of any compliance issues nor were any protective guards involved.
The operator was sitting in an Off Road Truck being loaded when Excavator swung around and struck the catwalk with the counterweight, shaking the operator on the inside of the cab.
Employee stated was attempting to place the bolter cable onto the scoop when a loop of cable struck EE in the mouth busting lip and chipping tooth.
While inspecting rollers in the impact bed under the slope belt, the employee got arm caught between rollers in the impact bed.
Employee was re-hanging cable on off-side(blind spot) of shuttle car in face(#4 entry cut), when the loaded shuttle car moved & bumped them. They did not seek medical attention until 07/10/2017, stating their back was sore. Employee has lost time, but returned to work on 07/12/2017 regular/full duty.
Unloading acid trailer and hose started to swell. Shut down operation and hose blew.
Employee was helping lift a piece of metal out of wash box and thumb got caught under metal.
Employee tripped over sump frame beam and fell, employee broke femur.
Employee stated that there was a power bump in the system and they were running to see what happened and twisted their back.
EE was on the property of the Stockertown Quarry and needed to pass a cement truck that was parked. As EE was passing the cement truck the driver of that truck pulled rapidly away from parking spot thereby sideswiping EE's truck & pushing it to the right. Truck damaged & EE's wrist hurt.
A powder employee was using an articulating loader to clean a blast pattern up for the blasting crew when the center pin broke causing the cab to tip backward. The operator experienced some jostling and received a minor injury to the neck. The employee returned to the clinic on 7/12/2017 and was prescribed pain medication.
Employee was working on machinery when back popped.
After fueling loader, tripped on fuel hose and fell off steps of loader
Employee stated was operating a haul truck and came upon a rock in the road. Employee got out of the cab and moved the rock by hand. Employee felt a sharp pain in abdomen. Was later diagnosed with a hernia and had surgery to repair. Began missing time on 07/27/2017.
Operator with fully loaded haul truck traveled down from upper loading area, along approximately a 1,000' long haul road with an approximate 15% grade. Operator noticed a situation with the brake system on truck. As EE approached lower storage area EE was unable to stop. Operator diverted truck against stored material, contacting stored boulders, overturning dump bed and stopping.
The employee was bending a roof bolt to be put into the drilled hole when employee felt pain in their shoulder while bending the roof bolt.
EE was hauling supplies in diesel scoop when hit a hole in bottom bouncing them into the top with their head, EE was wearing a hard hat. EE felt pain in neck/back region. EE went to doctor & diagnosed w/muscle sprain. EE continued to have pain, followed up with dr. and has loss time starting on 07/20/2017.
Employee started work shift at 4:00 a.m. 7/5/17. EE had went to do ee walk around and plant area checks. EE was walking between the Greens Mix tank and the Concrete Screw. EE hit head on a cross brace. Reported the injury to another ee who works in the same area with ee. Then reported to Safety Director@8:00a.m. EE was taken to the emergency room for evaluation for head injury.
IE (Temp. Employee) was backing roof bolter out from the face using a remote when the IE was struck by top coal/rock on the top of the head and lower left leg. The top coal/rock was approximately 3ft. X 8ft. X 0-3.5in. IE sustained a fractured tibia and fibula of the left leg.
Employee was digging in ultra fine pile with 980 loader ee got to much dirt in bucket which caused rear of equipment to raise up, When rear raised employee tried to quickly lower bucket which caused equipment to bounce hard. When equipment bounced employee was also bounced around. Employee did have seat belt on.
Employee was pulling plastic steps into place while working on a motor in the underground shop. The steps caught EE's left foot and EE fell to the ground. EE felt pain in left knee. Diagnosed with ACL and meniscus damage. LTA 8/21/17.
Employee was adjusting the door on the feed hopper at the crusher. The door has a handle to lift to raise or lower door. As the employee lifted up on handle employee strained a muscle in employee's back.
The employee was assisting others lift a steel frame. During the process the frame slipped out of their hands and fell onto the employee's foot causing a contusion.
Employee was working on a belt line. The employees foot slipped and they felt a twinge in their back. Employee finished shift and later pain worsened and they went to the ER.
Injured employee received a fractured lower left leg. Injured was bolting roof in the mine while in the man basket of the bolter. The connection on the machine between the man-basket boom and the equipment chassis failed causing the man basket & employee to fall to the mine floor. EMS was summoned.
An employee was in the LV712 transport van traveling to 12 shovel. When van hit a pot hole, employee experienced lower back pain. Employee traveled 1.5 miles to EE's assigned equipment at the 5150 before EE called HS3. On the evening of 7/4/2017, the employee returned to the clinic and was prescribed pain medication. Employee was scheduled off 7/6, 7/7, and 7/8.
EE was emptying a sulphur tray when EE felt pain in EE's back.
Employee was climbing on set ladder when ee rolled left ankle.
Employee was putting chain through a fish plate and was standing near rail car when a rail fell off and caused injury to their foot.
An employee was walking a 5-ft ledge of loose gravel to check the delays on a shot line when the employee lost footing. The employee fell against right forearm, which jarred shoulder and back. The employee requested medical care and cannot complete regular duties. The employee will be reviewing MRI results with a doctor shortly.
EE was kneeling down on steal grating to get some bolts out of the bolt bin and EE felt something in knee pop as EE knelt. EE did not seek medical attention until 8/2/17.
During a power move on the 10 Left section, the employee was helping drop a Hi-Voltage cable from roof of the mine when the cable hanger end caught the injured persons right index finger causing a laceration which needed stitches. In the following days after stitches this wound became infected and employee began missing work as of 8/1/2017
Employee reported severe shoulder/neck pain the day after hammering out the mill 03 feed system from bridged and plugged wet feed.
Employee was carrying a clip board, thermal temperature gun and radio in the right hand and using the left hand on the handrail. As the employee started climbing the second flight of stairs the employee felt a grind in the left knee causing instantaneous pain while stepping on the second step.
Employee was filling out pre-op/dust parameter sheet when a rock fell out between the bolts and struck ee on the head, shoulders, and/or back. The miner was not running at the time of the accident. EE received a broken nose, and 3 fractured vertebrae. The Accident Hotline was called on 7-27-17, to report on this accident to abate citation # 9104928, Issued on 7-27-17.
Rock broke between splitter and table causing a smashed finger between rocks.
Employee stated ee was going to load supplies onto the scoop on the 4 Mains section. In the process of walking toward the scoop the employee slipped on a roof strap which was on the ground. This caused the employee to lose balance, fall to the ground, and twist back.
EE was bolting on #2 section on the #5 Heading at survey station 8456 on the left curtain side of the bolter. EE was installing second bolt in the third row. During this process a rib rolled striking the EE on the right leg which resulted in a fracture.
Injured worker was exiting the cab of a front end loader. IW failed to grab the handrail and fell to the ground. Left arm was fractured. Handrail was inspected by both the operator in pre-shift inspection and company personnel. It was found to be in proper working order.
Employee was helping change the paddles on a scrubber. Employee was standing on the frame or the scrubber tower guiding the scrubber's into the chamber when employee lost balance and fell on to catwalk about 5 feet below. Employee was wearing fall protection but was not tied high enough to dampen the fall.
Employee was removing bolts from a Flange connecting 2 pieces of pipe .The incline pipe going to the gravity box was being held by a chain connected to a trackhoe, EE took all the bolts out of the flange except the bottom bolt. While removing the last bolt the side of the pipe not being held fell pinning a finger between the flange and a concrete block.
Index finger bent backwards while pulling cable off when cable suddenly started going the opposite direction due to cable on tugger was wedged in a spot and started backspooling. Employee went to Doctor on 7/25 and was put on restrictions starting 7/26.
Employee was stepping off of the track mobile up at the load out building while turning to step away employee's left foot slid out on the rocks causing employee's right leg to twist and buckle. EE fell on right knee with all employee's weight. EE went to doctor the following morning as the pain got worse. EE was put on restrictions for knee pain.
Employee was inside a head-box cleaning it out. As employee went to exit the box employee felt a strain in employee's back.
Employee walked between 2 mantrips when the operator of the mantrip in front engaged tram switch and the mantrip took off pinning employee between the 2 mantrips.
Changing hoist rope and lost control. Fiber rope attached to the end of the hoist rope. EE left hand was injured when the rope gave way and ee fell to the ground, injuring left shoulder and knee.
The employee was removing steel roof beams and materials. While attempting to remove a beam, the injured was struck on the back and neck areas by a piece of falling steel.
Employee was in the process of fixing a broken cat pad when a section of rock fell from the rib striking employee in the lower back causing a fracture to lower back
Employee was on knees and reached to get a crib block, as ee twisted upper body to place the crib block on the tailpiece, ee felt a pain go down through right leg. The employee continued to work and was later seen by a chiropractor. The pain in leg persisted and ee was seen by a neurosurgeon on 8-11-17; it was during this visit the neurosurgeon put ee off work.
Employee was boarding a Cat 777 Truck when employee felt pain in their lower left leg. Medical treatment followed.
While moving density logging tool, employee slipped and tool fell from hand injury to tendon in arm.
Bushing a piece of product and felt a pull
Worker was attempting to weld on a piece of c-channel iron placed on a welding table. When leaning over they knocked the iron off the table and onto their right foot. The impact broke 2 bones in the arch of their foot.
EE was walking towards the rear of the continuous miner checking the center line when a piece of rib rolled out striking EE on leg.
Fuel delivery driver was climbing down a ladder and slipped and jumped down off the bumper of the trailer injuring their mid back section.
On July 19, an employee was walking down a belt ramp when EE stubbed boot toe on the metal grating and twisted right foot/knee outwards. The employee did not require medical care. On July 31, the employee stated knee had not reduced swelling and requested medical attention. An MRI has been scheduled. The employee cannot climb and squat.
IE was attempting to put a dragline tooth in place. IE dropped the tooth and it struck an adapter on the ground. The energy from the two pieces of metal striking each other resulted in a sliver of metal striking the IE in the right shin. Metal was removed by a doctor.
While unloading tires from a 53 ft trailer to the shredder, the employee felt a growing pain in EE's abdominal area. When inspected area employee notice a bump inside EE's navel. Employee was seen by doctor and was diagnosed with a umbilical Hernia without Obstruction and without Gangrene. Return to work that evening with Limits of 15 pound
Hit back of head while operating a Terex Haul Truck after it stopped on unstable ground and the back end sunk down and truck tipped back. Didn't seek or receive treatment until 8/01/17 after having head aches and neck pain. Treatment was received at Back to Health Clinic in Keizer
Employee was in the process of assembling a hydraulic dipper together in the dipper shop. While walking around bowl the employee stated they slipped on a 1"" x 1"" piece of metal with left foot. After slipping, the employee caught themselves with right leg straining hip. Incident was not reported until 7/21. Restrictions not given until 7/25.
EE sprains right wrist while trying to unplug the power cable of bolter from the j-rack, right hand slipped on the plug and EE felt a pop.
Was moving the man lift basket. When it stopped, it came to rest against the handrail of the processing plant catwalk. This pinched the skin off at the tip of the finger.
Employee stated EE was holding a knocker while attempting to repair a splice while another employee struck the hammer on the knocker. The employee missed the knocker and struck the injured employee on the left index finger.
Operator was cleaning material from floor from broken bags. Operator got cement/lime powder on leg and the hot weather caused sweat to move lime/cement down into boot. One leg had on short socks and did not stop sweat. Operator experienced a lime burn and needed treatment. Burn was not reported until 7-21-17 and then medical treatment followed.
EE was pulling self out of a rail car and heard shoulder pop. EE did not seek medical attention until 10/24/2017. We are filing this 11/2/2017 within the 10 day reporting window.
Employee was holding a rock on table another employee spun a rock to the table smashing their fingers between rocks.
Employee was packing crib blocks when foot got stuck in the mud causing ee to fall resulting in a fracture to right ankle.
An employee was working on pipes when they mistakenly opened the line causing a small section of pipe to strike their arm and face.
Employee was handling material. While attempting to lift material to a platform, some material shifted and began to fall. The employee reached to catch the material injuring their left arm.
Miner was pounding in a track pin with a sledge hammer when a splinter broke off from the pin and embedded in miners left leg, above the knee on the inside of the thigh.
On 7-13-2017 employee reported lower back pain as a consequence of placing empty bags on loading packer spout.
EE was placing bolts on top shell for bottom shell securement. EE was holding top shell bolt with right and on bolt face. While utilizing an impact wrench to tighten a nut EE engaged the wrench, the bolt spun and the bolt face grabbed EE's right hand, crushing EE's thumb against the crusher bottom shell wall. EE sustained a fracture to thumb.
Operator was handing bulk bags, when removing bags from hooks EE got hand caught between strap and bag. Was not following proper communication with forklift driver and other operator.
Employee injured left small finger when it was pinched while loading a repair part.
The IE felt pain in the lower back when they bent over to lift shovel trail cable. The IE went home at the end of shift assuming it was an aggravation to a previous back injury. On 07-13-17,the IE called wanting to see a doctor. Medication was prescribed and was referred to an ORTHO. On 7-14-17 the ORTHO evaluated the IE and ordered a MRI for 7-18-17 revealing a herniated disk.
Employee was putting feeder pads on primary feeder. Contusion of right hand-palm below thumb.
EE was working second shift on July 10th, the first shift back from vacation shutdown. Approximately three(3)hours into the shift, EE twisted right knee and heard a loud pop as EE was transferring a bundle of glue from the rear of the bolter to the front of the bolter. An MRI revealed a torn Meniscus. EE started missing work on July 21st.
The miner was fueling the haul truck from the fuel tank. The miner turned around to talk with another associate and tripped over the large orange wheel chock falling on left knee cap. The miner had surgery on December 7, 2017 and is expected to be off work two to four weeks.
The employee walked near an open blast hole when the collar of the hole caved, resulting in the employee's foot and leg falling into the hole. The employee grabbed the discharge auger of the truck, which was positioned to load the hole. The employee received 1st Aid for a shoulder strain and returned to work. On 8/7/2017, the employee was placed on leave by the medical provider.
Employee was taking a sample from the caustic line in the blunger building at the mine plant. Crystallization had built up in the line causing reduced flow. After taking the sample, the employee reached down to close the valve right as the crystallization broke free in the pipe causing caustic to be sprayed out.
Employee twisted knee resulting in a torn anterior cruciate Ligament while climbing down from a flatbed trailer.
Injured was struck by the battery end of a coal hauler, as employee was working on miner cable along the rib-line. The injured suffered bruising along their side with no fractures or other injuries.

Employee was giving a truck driver directions while on the pack house dock. The employee stepped down from the 3ft, 3inch high dock, lost EE's balance when EE's feet hit the ground, and fell onto EE's elbows and rolled over, impacting EE's right shoulder. EE was treated with first aid until 7/26/17 when EE was placed on restricted and given meds.
Employee lost balance causing ee to apply all weight to one knee to regain balance. Felt pain in knee directly after. Took to Doctor same day, released full duty with restrictions.
Miner was standing on a ladder cleaning the grizzly in dry 2. The ladder twisted causing miner to loose balance and fall approximately 5 feet to the ground.
EE went to align anodes in a cell. As EE stepped off the grading onto the cell line EE felt left knee pop.
Injured EE and witnesses were changing a bend pulley on 50 conveyor. While performing the repair, the mast on the telehandler appeared to be lodged against the conveyor frame. When injured EE reached inside of conveyor frame to hook chain on pulley shaft, the mast moved, pinching left arm between conveyor frame and telehandler mast causing injury.
On 7/6/2017 at approximately 2:00 pm, service technician was riding in the jumper seat of a man trip at the Bailey Mine (Crabapple). When exiting the man trip, the employee underestimated the distance to the ground and hyperextended right knee. The injury was reported late afternoon of 7/10/2017.
Employee was removing the bolts from the motor base of a 500 hp motor using a wrench and 20 pound sledge hammer. As employee attempted to strike the wrench with the sledge hammer, ee missed the wrench causing left index finger to get struck between the hammer handle and wrench. This resulted in a broken left index finger.

Employee slipped on wet concrete and broke left fibular.
EE was lifting the secondary crusher suspension bar into place when felt a strain in lower back. The incident occurred on Monday, July 3rd, 2017 and EE reported the injury on July 7, 2017. EE received treatment from a chiropractor and is working modified duty.
The employee was climbing down the FVL on BE-05 to take a lunch break when ee stepped off the ladder onto the 1st. elevated platform, ee felt a strain in left knee and heard it pop. After going home that evening the knee had swollen and was in pain. The next day the employee went to an urgent care facility and was diagnosed with a sprained left knee.
IE was attempting to connect the discharge hose to a pump when the pump capsized. IE was struck by the pump resulting in fractured ribs.
Crew was replacing rotation unit. Unit slid on frame into hand pinching hand momentarily causing contusion.
Employee was attempting to remove teeth from a loader bucket. When they struck a stuck tooth with a soft-face hammer, a piece of the tooth broke off, lodging in their left leg. Employee required surgery to remove the piece of metal.
Emp claimed inj back on Fri. 7/28 but did not report it until Mon. 7/31. Emp was oper. a Komatsu 400HM haul truck and hit a pothole in the haul rd. causing inj to lower back. This report was originally submitted via paper copy, and this report is being generated to make amendments per MSHA field office. Original was submitted 8/8/17.
Employee stated they went to the end of the track in the #3 entry to load structure into an insert, as they were walking they stepped into a hole and rolled their right ankle.
A Drill-Helper was attempting to load another stick of drill pipe into the Pipe-Arm Loader on drill rig #418. The Drill-Helper was holding a 36"" pipe wrench with R hand & operating the remote control with L hand, while dragging the pipe into the cup the R pinkie finger was caught between the pipe wrench & hydraulic ram cylinder causing an avulsion to underside of pinkie.
EE bent over to remove wheel chock from trailer they were working on. They felt back pain when they bent over.
The employee was moving 480 volt power cords during plant set-up and strained left bicep. 1st aid treatment at that time. On 9/10/2017 pain was getting worse, ee was taken back to the doctor where ee received lifting restrictions and set-up with physical therapy appointments.
A mobile maintenance mechanic was coming down the ladder of a haul truck. As foot contacted the ground, ee right knee buckled.
Employee was dismounting dozer when EE misplaced foot and lost grip resulting in an ankle sprain.
Employee was utilizing a small/dremel type tool to grind a shaft when a piece of debris entered employee's left eye. The employee did not report the injury until the following day. Employee received treatment at a local clinic and was prescribed eye drops.
Employee was assisting another employee pick up a 55 gal barrel that had fell over. As employee was pulling on the top, felt a pop and a sharp pain in left bicep muscle near the elbow. Employee was seen by a Dr. and diagnosed with a torn bicep muscle.
Employee was stacking stone on a pallet when employee experienced pain in the lower back and groin. Employee had been feeling symptoms for two to three weeks prior to reporting injury but the pain became more intense so employee reported it.
The employee was placing two 7 foot scaffold legs to be secured into place when the first leg slipped out of the right hand and hit the thumb on the left hand as they held the second leg. The impact caused a contusion injury to the tip of the left thumb.
Employee was standing on scaffolding inside a tank. EE was removing a sand-blasting hose to hand it to another EE who was standing on a platform above. The hose caught on the scaffolding plank EE was standing on causing it to slide off one side of the piping it was positioned on. EE's left arm got caught on a scaffolding cross brace.
Employee was operating the walk behind TX 1000 Dingo in #4 re-claim tunnel. As the employee transitioned the machine from the concrete apron to a downhill sloping sandy surface, they were flipped over the top of the machine landing on their face. They suffered a laceration to their face requiring 16 sutures.
Employee thinks EE hurt back while working on the C-17. EE did not seek treatment until 7/28/17.
While the EE was shoveling material the shovel hit the floor then EE felt a pain in upper rt Arm/Shoulder causing a muscle strain.
Employee was getting out of loader after EE had finished loading the dump truck, when the last step on the loader bent inward towards the loader causing employee to fall backwards and to strain right knee.
EE was holding metal piece while tightening bolt with other hand. When the bolt was tight ee moved left out of the way and finger was cut on a burr from the metal ee was holding. Left index finger
Cleaning catch box, a rock was jammed inside. Another employee threw a rock down into the hole while the injured employee reached down to get a different rock. Rock struck injured employee's hand.
Employee was installing a 20"" ball mill feed elbow. One threaded studs fell out because the weld broke. Employee went to the repair bay to clean up this 1"" threaded stud and to grind off the weld left over on the bolt. Employee was holding the bolt in one hand and the grinder in the other. The grinder bucked and blade made contact with thumb
Employee was traveling down a ladder, tripped and struck right knee on block ... reported injury on 7-28-17. Emp went to Clinic and was put on restrictions. Emp doing regular job (missing no time) but working with-in restrictions.
Contractor employee was repairing a sump. EE cut the discharge hose to length with a utility knife that slipped and made a laceration on left forearm
EE reached down to retrieve a grate and experienced pain in back. EE reported the pain but expected the decreasing soreness to go away over time. EE did not pursue or request further treatment until 8/16/17 when EE reported increasing pain in back from the July incident. Lumbar paraspinal muscle spasms identified. Restrictions placed/PT and meds prescribed.
While the employee was working on the bin feeder, they tripped, falling backwards and hit their right elbow.
The injured employee was operating a forklift on a gravel drive / parking area to empty a material tube. When returning to EE's area the forklift was struck by a semi-truck traveling at approximately 5 mph. The impact jostled the lift and operator resulting in a cervical and lumbar strain and restricted duty.
injured was hanging a curtain and felt a pain in shoulder area, but continued to work. Pain continued to progress and injured had an MRI scheduled and on 8/16/2017 injured was diagnosed with a tear in the muscle in the shoulder and placed on restrictions.
Employee was placing a gearbox (5lbs) on a reactor and when EE tried to stand up EE felt a tightness in lower back.
Climbing into truck body and strained back
Employee was jack hammering buildup in cement elevator, when ee felt strain in lower abdomen. EE did not initially report, but as discomfort persisted, reported on 8/4/17. Physician diagnosed as thrombosed hemorrhoid. Procedure to drain resulted in restricted duty.
The employee parked loader at the ready line parking lot & was exiting the loader to go clock out as the employee was at the end of shift. While exiting the loader, stepped on the ground felt a pain in right foot. There were no adverse conditions or hazards associated with the area employee exited at. The ground was dry & even and employee used three points of contact.
Employee was walking up stairs in the Hydrate Area and felt pain in left side and pain in lower back. Requested medical attention on 8/13/17.
Employee rolled ankle when ee stepped on a Rotex rubber ball while collecting a plant production sand grade sample.
Injured employee had just completed rigging a load on the 300 ton Link belt crane. EE made eye contact with the operator and asked if EE could come and talk to EE. While up on the deck talking to the Crane operator EE fainted and fell to ground and broke left clavicle. Employee was taken to Hospital where EE was found dehydrated and taking 3 meds for blood pressure.
Employee was operating a loader when a Haul truck backed into the loader jostling the driver in the seat and hurt lower back.
EE was manually spinning the sand pump shaft by using hands to rotate the v-belts on the pump to check impeller clearance. EE finger got caught between the v-belts and motor sheave as ee was turning the belts
Employee was shoveling clinker out from under a belt conveyor counterweight. EE right foot slipped on a piece of clinker, causing ee to jerk to catch self and twisted back in the process.
Employee was installing a new tail roller on the fines collection belt of the McCloskey 733 trommel. The roller slipped while installing the bearing on the shaft. Employee's left thumb was pinched between the shaft of the roller and the belt alignment adjustment bolt.
Employee was dropping off a loaded trailer in the truck parking lot. While cranking the dollies on the trailer down, a bee landed on the employee's right side. This resulted in the employee being strung by the bee and the employee having to self administer an Epipen as employee is allergic to bee venom.
Employee was repairing bent leg of dredge suction cage and was heating it with a torch. Second employee reach to move it and grabbed the hot piece of metal with employee's bare hand, Employee received Less than 1% second degree burns to employee's right hand.
Miner was picking up a piece of limestone and they felt a stab sensation in groin area, left side
Employee stepped out of ticket booth and turned ankle on steps. EE twisted ankle and hard hat cause a head laceration.
Employee was removing strap and shackle, bar or strap moved, pushing left hand into the sheet metal side of the cooler. Causing laceration to 4th finger on L hand .Employee received medial treatment, stitches and x-ray, on 7/20.17 immediately after injury occurred.
Employee bear hugged injured employee around the chest/arms and suffered a chest strain.
EE was pushing wheelbarrow through mud and it tipped over. EE went to brace themselves and landed on their right thumb. Fractured right thumb. EE released to restricted duty which the site could accommodate.
A driver activated the wrong truck access system and activated the one that the injured employee was working on allowing to lower down striking the side of employee's hard hat in a downward motion. The injured employee only asked for treatment and was given on 7/24/17.
An employee tripped and fell while loading blast holes, causing a hip joint sprain. The employee received work restrictions from a healthcare professional.
Employee operating the Quarry End loader was working into a pile of shot rock when they were jarred and then felt a pop in their left shoulder. EE was sent to Occupational Health Department and was placed on restrictions 7/19/17 that will not allow them to perform full duty.
Employee showed up at work 7/19 complaining of left shoulder pain. The previous day, the employee had been a hole watch (conf space) assisting and monitoring employees accessing the conf space. When EE left work at 5 pm, there was no complaint of injury. We took EE to the Doc, Doc put EE on light duty and gave EE Naproxen/Flexeril.
Removing fittings with a crescent wrench. While pulling on the wrench the employee felt a pop in left elbow. Initial diagnosis is muscle strain.
EE mechanic jumped in to help another employee drag a two inch suction hose down the longwall face. EE lost balance and fell striking forehead on the toe of the roof support. As a result of the fall, the EE received a laceration to the forehead above the eyebrow requiring four sutures.

Employee was placing a strap under a 36 inch corrugated drain pipe down in a ditch
The employee was removing a starter bucket from MCC 3 for the #3 Kiln Heat Exchanger screw. When they began to remove the bucket a flash occurred causing 2nd degree burns to their left and right hands.
Was dislodging a rock with pry bar, rock dislodged and pushed bar and hit hands on feeder box.
While descending stairs employee felt a pop in left knee. Employee has a strained muscle in the back of knee.
Truck operator injured shoulder as EE was climbing up access ladder of CAT 773.
Two employees were loading 10' lengths of stainless steel shaft from a vertical turbine pump into a loader bucket. They were performing two man lifts. At some point IE started lifting them alone and back popped and felt a pain in lower back. IE finished loading the shafts and then came to the surface and told the shift foreman what happened.
Employee was struck on the left side of the head with stone or dirt.
When lifting a bucket of material sample, of around 35 lbs., to put it into the back of a truck, EE felt a pain in right knee and cannot keep balance and fell.
EE was stepping out of port o pot and rolled ankle
Getting off Excavator employee (who is over weight and has arthritis in both knees) fell on EE's bottom and had trouble getting up. Employee used 3 points of contact and when EE's rt foot touched the ground EE released EE's hands and EE's knee went out.
This is being filled due to a citation issued during a Part 50 Investigation. On 7-17-2017 the IE was removing trash from a xc and tripped over a concrete block. While trying to catch themselves from falling. The IE extended arm out and came in contact with the coal rib resulting in pain to shoulder. The IE worked until 2-28-2018 and then had shoulder surgery.
Employee was unloading 10lb sample bags from a flat bed trailer one a time. Employee reached to grab one and started to pull and lift not realizing their were three bags tied together. employee felt a strain and pop to their wrist.
Miner was fueling a haul truck and was walking from fuel station around the truck, slipped on loose gravel causing miner to lose their balance and strain their back. Miner refused treatment until 7/24 when aching didn't recede, restrictions placed then.
While the employee was moving a 3 inch sump line, on one end of the line was a rope, that the employees used to assist with moving the line across the tank. The line fell, causing the rope that the employee was holding on to pull and jerk on the employees shoulder.
Bearing went out on south bag house fan. North bag house was still running causing a draw on the south. Employee tried to stop it with a pipe wrench. Wrench slipped off shaft and struck employee's left arm.
Cut right ring finger when it was caught between a sharp edge on an engine block and a piece of pipe while lowering engine block to the ground.
Ee was prep to clean the A/C of the control tower with the water trk but while exiting the truck ee stepped on a rock and sprained left knee. The ee sought medical attn on 7/24 and was diag with a sprain and released w/o rest. MRI results were obtained which revealed a hairline fracture of the femoral condyle which made this as a med reportable injury as of 8/7/17.
Employee stepped in a soft spot of sand while walking and felt pain in their right knee.
The employee slipped as EE was walking up an incline. When EE slipped, EE fell on EE's right ankle causing a broken tibia.
Muscle pain in right arm resulting from pinched nerve.
While roof bolting employee got right ring finger caught in the jaws amputating the tip of employee's finger.
While pulling electrical wires through conduit employee hurt right shoulder.
Employee was using a scaling bar to scale down loose material. The loose material then caught on the scaling bar causing the bar to hit the employees leg/knee requiring 6 stitches to their leg.
Employee was pulling a rock from the sorting line to the discharge line and got finger caught between the rock and cross brace smashing the left ring finger.
The injured employee was walking across the railroad tracks towards the employee building and began walking up a small hill when they stepped on uneven ground and rolled their ankle. The employee felt a pop in their left ankle and had swelling.
On Thursday morning EE was on way to grease shed to pickup supplies to service haul truck. We had heavy rain the night before and into that morning. From all the rain there must've been some wash out around the shed. EE walked into puddle and twisted left ankle.
Miner was using a ¾ inch ratchet to operate a gate on the S3 structure. When the miner pushed down on the ratchet, their pinky finger was around the end of the ratchet handle and the ratchet slipped out of the modified shaft end causing their finger to be caught between the guard and the handle.
When removing the cutting edges from a bucket, the employee was holding the bolts up with the hammer when the bolt slipped off and the sharp edge of the bolt head cut the tech's palm side middle finger.
Mechanic was using a 14oz. ball peen hammer and a punch to remove 3 stripped bolts. Employee used the hammer and punch off and on for approximately 1 hour before the bolts were removed.
Employee was working on the primary, ee foot was over the kick plate on man lift, other employee lifted the basket pinning foot between kick plate and belt
Working on primary crusher, removing belt clamp, belt clamp still under pressure, when the clamp came loose it pulled back knocking the ratchet from employee's right hand, wedging hand between belt clamp and support bar.
Employee was pushing down on pry bar with a closed grip. The pry bar slipped causing the load to fall on top of the pry bar. Due to the closed grip the employee's finger was pinched between the concrete and the pry bar. The pressure caused the tip of the finger to split. Leather gloves were worn.
EE was opening railcar lid, as EE raised the lid, the lid got stuck on the spout causing the lid to bounce and hit the top of EE's right hand.
Injured miner was changing the outer rear tire of a semi-trailer when another miner lowered the floor jack. The trailer came down and pinched the injured miner's hand between the tire and the trailer.
Removing Liners from Roaster South Mill Feed Chute. Welded picking eye to the liner and used a hoist to remove. Weld on picking eye broke which caused the picking eye to hit injured employees left index finger causing a laceration and a chip fracture.
Employee was installing belt structure when they stepped in a hole dropping a bottom belt roller on their left ankle. Employee later sought medical attention and was diagnosed with a fractured ankle. This is a medical reportable incident.
Employee was assisting with pulling pipe from dredge onto shoreline. Wire cables were hooked to pipe and they were using a loader to pull pipe with. When loader began to move, there was slack in cable that tightened quickly and struck the employee on the left leg causing them to fall and then cable ran across their right arm and back.
Employee descended a ladder (face first) and felt pain in right shoulder.
While backing the fork truck and looking over right shoulder, the employee felt a ""snap"" or ""pop"" in right elbow, including numbness in the middle, ring, and little right hand fingers. Employee did not hit or make contact with anything. Same day medical treatment included use of sling and restrictions. Reexamined by medical professional and fully released on July 13.
Worker fractured right long-finger (metacarpal). EE was cleaning travel way at the scalping screen when a rock fell from the tunnel belt striking employee's right hand. EE's hand was rested on steel railing at moment of impact.
The employee was cleaning out a plugged chute in the dolo crushing area. The employee opened an inspection door and hot material came out and contacted the right side of the employee's face causing first and second degree burns on the cheek and ear.
Crew was turning the 7ft short head crusher bowl in order to remove it for repairs. Injured EE was pushing on a pry-bar to rotate the crusher bowl. The bar slipped out of the cylinder hole in the bowl and the employee heard a ""pop"" and immediately felt pain in the left elbow. The employee was taken to the emergency room for evaluation.
The injured was in the process of closing a set of equipment air lock doors when EE was struck by the door.
Employee experienced pain in ee neck after equipment. On 7/11/17 the employee was given restrictions but started ee 7 days off.
While attempting to remove to remove a shiv the pry bar slipped smashing the the tip of the employees right ring finger between the bar and a tension shiv.
Employee was driving a mechanic truck and drove through a rough area in the mine road causing employee's back to be strained.
Employee was using a dead blow hammer to install pipe in a trench. Employee attempted to swing the hammer to strike a 2x4 piece of wood. The hammer deflected off the side wall of the trench striking the employee in the left hand fracturing their middle finger.
Preparing to repair a dump belt splice on a 18 ft x 72 inch conveyor belt. The belt was locked, tagged and verified then the drive belt guard was removed. Injured was using both hands to pull & push on the v-belt drive belts. EE caught index finger between the v-belt and the pulley. Fracture of the distal phalanx left index finger.
Employee was walking down the stairs of C6 conveyor when one (3rd step) of the stairs gave away and employee fell through the stair well.
The injured employee was walking to turn off the augers power switch. Employee twisted employee's ankle when employee stepped over a 6 inch steel pipe.
Employee was assisting with changing flights on F26 Longwall face at the headgate. A rock fell from the roof / face and struck the injured on left foot. The shields were pulled in to the face in this area. Diagnosed with fractured left 5th toe on 7/24/17.
Employee was retrieving equipment following a shot. Employee bent over to check a video camera before turning it off and felt an intense pain in employee's lower back. Employee reported pain while walking and getting in and out of vehicles.
A drill operator was climbing the steps on the 11R drill when knee buckled causing ee to hit the same knee on the steps. EE received a minor scrape and some knee pain as a result. On 7/7/2017, the employee returned to the clinic and given restrictions.
Employee originally felt a pop in shoulder on 6-29-17. EE did not receive any restrictions from the doctor. In a week EE was still hurting went back to the doctor on 07-06-2017 and received PT for 3 weeks. Employee was in a man basket and reached out to a light fixture and felt a pop in the shoulder.
An employee was assisting a customer truck by cleaning off material from the tailgate. The employee notice the tailgate was not latched, so ee tried to close it and caught the tip of finger between the tailgate and the bed of the truck.
Employee had just completed repairs to pipe on dredge and was walking stepping down off of pontoon boat to the ground and felt pain in right knee. Employee did not report injury until 8/1/2017 and sought initial medical treatment on 8/2/2017.
Employee LOTOTO K1 coal mill rotary feeder. Employee unclutched hand wheel and tried to spin it to free up blockage. Employee received laceration to left hand, between the thumb and index finger. Employee was not wearing gloves. Hand wheel looked to have a rough edge on the inside from casting. Employee received 3 stitches and tetanus shot, then returned to work on light duty.
Employee received a laceration to their left thumb from broken glass. The laceration severed a tendon which was repaired surgically through an outpatient procedure.
Pulled muscle in lower back.
Employee was attempting to break up compacted material inside extruder hopper. Employee used a large auger part to strike the side of the hopper. Employee smashed employee's finger between the auger part and the hopper.
Closing gate at end of day, heel got stuck under bar on gate, lost balance and fell.
The Injured miner was exiting a forklift and when turning to get off felt a sharp pain in the middle of back on the left side
Employee had a loader bucket on a goose neck trailer, and threw a chain and binder over the bucket to chain it down. When employee threw the chain employee felt a share pain in employee's right elbow and forearm.
Employee attempted to return a crowd belt tensioning wrench on its hanging hook. As employee attempted to secure the wrench, it missed the hook and the wrench fell on the employee's left index finger, resulting in a fracture.
EE was tasked with opening a lime chute slide gate. The gate was stuck and EE had to use a sledgehammer to work the slide back & forth to get it unstuck. While doing this EE's right wrist started to hurt. The pain continued for a few days when EE was treated @ the local Occ. Health for a wrist sprain. Dr. Rx modified duty, splint, and Rx meds.
Employee was operating the #272 CAT 992G Loader in the West Pit. The employee was doing clean-up in the #8 Shovel position. The employee stated that they contacted a few boulders causing the loader to rock side to side shaking the employee and causing a strain to the employees left arm/bicep.
Summer help were chipping and cleaning handrails on catwalk. Emp walked by area and stopped just as a piece of material bounced off catwalk and struck ee in the upper lip. Laceration / One Stitch
While cutting a Gradall shank with an oxygen acetylene torch, a piece of molten metal flew into the employees ear.
EE was asked by operator to re-install a ripper tip retaining pin. The pin had walked out of bore in ripper shank. EE struck pin with hammer to realign. When hammer struck pin, the impact caused a piece of metal to break off and was projected into EE's chest.
The employee was installing a lower tool bushing in the primary crusher Rammer E68 hammer. EE was using a (hand held) hammer to drive in the lower tool bushing retainer pin. One of the times EE struck the pin sliver broke off the end of the 1 ¼"" diameter lower tool bushing pin, and embedded in the employee's arm.
Sloped site conditions. Slipped and lost balance. Stepped into a rut. Twisted right ankle.
Employee was walking from Wedron #3 product elevators towards the maintenance vehicle when employee slipped in area covered with water and algae. Employee fell striking the back of employee's head resulting in a cut requiring 5 stitches to close.
IE was installing monorail hangers for the pump monorail. IE drilled the hole & installed the bolt. The drill box was raised to extend the second stage. IE had placed hand at the top of the first stage of drill mast frame. As IE lowered the drill box IE's fingers were caught between the second & first stages. IE broke one finger and dislocated another requiring surgery on 8/4.
Employee was working outside underneath a conveyor line when ee felt a bug crawling under pant leg. EE tried to kill it but it stung ee before ee could. Having a bee sting allergy and having seen it was a bee, employee administered an emergency epi-pen that ee kept on site.
The injured miner was placing an opened bag of Portland Cement next to the project, when a plume of the power like contents of the bag struck EE in EE's left eye. The miner washed EE's eye out at eye-wash station and received prescription pain medication at emergency room. The employee returned to work the next day.
Employee was tightening driveline bolts with a hand ratchet and it slipped hitting EE in the upper lip causing a laceration to the lip.
Driller smashed right ring finger while using a hammer to loosen the bit from a drill steel. The hammer had a glancing blow, allowing driller's hand that held the handle to strike against the bit.
Employee was moving fly pads and piece of draw rock fell from roof and struck EE in the face causing a small laceration.
EE was removing zip ties from hydraulic hose with a pocket knife and cut their hand. EE was taken to the emergency room where they were treated with one suture and tetanus shot.
Employee was walking back from the feeder to their car when they twisted their right knee. Employee was diagnosed with a torn meniscus.
Employee was picking up or removing 8 inch Tee from shipping container when the Tee shifted cutting the employee's left thumb requiring stitches to close the cut.
Employee was cutting a hose with a utility knife and cut middle finger requiring 6 stitches.
Employee cut between right thumb and forefinger while picking up gasket material.
Putting up cable bolt and cable bolt bent pulling arm with it straining right shoulder.
Employees was holding the nut / bolt on the bottom end with a wrench so that it would turn in the frame so it could be tightened from the top with another employee with a ratchet and socket. EE was overpowered and lost grip of the wrench, it fell off the bolt and EE's hand and wrench went forward and ran across the metal sheet cutting the tip of right pinky finger
While tripping out rods, the employee was moving the AQ spacer pipe from on top of the drill pipe to the storage area under the drill pipe. The AQ rod missed the inner most cross member and levered up pinching the employee's hand between the AQ and the drill pipe. Employee's middle finger was lacerated requiring 4 stitches.
EE was working on a belt at the vessel ref while using a box knife, the knife slipped and contacted the EE leg (thigh).
While handling miner cable employee stepped backward striking the back of EE's head on a roof mat that was hanging down on the corner. Resulted in six sutures.
Employee was punching out a hopper, that had stopped up, with a rod and felt discomfort in their right hand.
Employee was replacing cutting edge on a Cat 988 loader, when removing a bolt employee grabbed the head of the bolt and cut left ring finger requiring four stitches to close.
Employee was unrolling a tarp on a train car and went to loosen a ratchet strap, while doing so EE released the hand crank which struck EE in the lip. EE received 3 stitches.
REPORTABLE ONLY - Employee was exiting a man bus when employee stepped off the bus into a cross cut employee injured employee's foot resulting in fracture to employee's right big toe.
Employee reported to line-out meeting at beginning of the shift and was attending a safety presentation that was to be 1 1/2 hours in duration. 30 minutes into the training, the employee was not feeling well and was going to go get some fresh air. The employee stood up and fainted and dropped to the floor. This resulted in a non-displaced hairline fracture of upper jaw.
Collecting rail road ties for a contractor the employee climbed on the pile to get some short ties. When coming off the pile, the footing was unstable and they stepped back off the pile directly onto a large nail with their left foot. The nail went through the boot and punctured the foot.
Injured was assisting in removing a pipe flange bolt. Injured reached for a combination wrench that was falling off the bolt head to put it back on the head. As EE reached for the wrench the co-worker loosened the nut with a ratchet wrench. The combination wrench reacted to the movement of the ratchet and injured's right middle finger was pinched between the pipe and the wrench.
While working to clear a bridge at the primary crusher of the portable plant the employee was using a bar to try to poke material free and smashed finger between the bar and the crusher. Employee did not inform management at the time of injury. Employee later went after work to ER and received 4 stitches to finger. Injury was reported to management on 8/21/17.
Cutting conveyor belt and the knife slipped and cut arm.
While leaning foot on warehouse chain, associate went to lean elbow on wall, chain hooks broke, associate fell approximately 3'. Associate sustained abrasion to back of head and laceration to left elbow requiring 3 stitches.
While the employee was operating the remote control for the Continuous Miner, a rock fell from the roof and struck employee's left thumb. Diagnosed with a hairline fracture of the left thumb 7/27/17.
Employees were installing outrigger pads for placement, as they were taking the pads out of the racks on the crane the employee placed their hands under the pad instead of using the designated handles. As they pulled the pads out of the rack the employee's left index finger was pinched. This resulted in a laceration and broken left index finger.
Employee was closing the door to the loader at the end of shift, had lunch box and bags in hand, turned around to shut the door and caught tip of left middle finger and artificial nail in the door.
EE removing a pinion from Cat Loader when the part hit employee on the face causing a laceration to left eyebrow. EE received 4 sutures and was released back to full duty.
***Per phone call: Employee was walking up stairs, caught boot on top step and fell to working surface. Lacerated elbow.***
Overhead wires knocked down by dump truck. Lines on ground were de-energized. One line on pole appeared energized and was not knocked down. De-energized line came in contact with energized line. Flash occurred causing a burn.
EE was using a hammer on belt splice when a piece of metal came off hammer & went into their hand at the knuckle between the index and middle finger. The metal was removed & they received 3 stitches. EE has no loss time, EE returned to work.
Operator backed against stockpile and material was unable to dump properly. Assumption is material may have shifted bending the hydraulic cylinder and haul truck then slowly laid on its side. Diagnosis is transverse process fractures.
The employee was walking under a piece of belt guarding and hit employee's right ear on the guarding, causing a laceration. 1 suture was required to close the wound.
While attempting to break a plug on a wheel loader at work, the employee slammed hand between the ratchet wrench and the inside of a steel rim detaching the whole fingernail and causing a laceration to the left pinky finger.
During the bolting cycle as the employee was looking up to release the leg feed on the jackleg a rock fell thru the section of wire ee was bolting up and contacted the right side of nose causing a laceration. EE was seen by site medic, referred to ER, one suture was required and a full work release was issued making this a medical reportable.
Employee energized air hose, the opened end of hose whipped around and the hose fitting struck employee on the head. The hose caused a laceration to the employee's head. Employee was released to full duty.
Two employees working in a 125 foot man lift, replacing tin siding on Prep Plant. Injured was holding tin, man lift lowered, when stopped it bounced, causing injured employee's right hand to hit safety rails on man lift. Top of tin cut right middle finger.
Operator was shoveling material into White Rock Elevator in the Dryer Pit The Operator was investigating why the vibrator was not working and grabbed the hose that supplies air to the Vibrator for the Elevator. When the operator grabbed the hose it pulled the lid to the Elevator down onto the operator's wrist. The operator experienced a contusion to the left wrist/arm.
A rock struck an employee in the mouth chipping/cracking their front tooth in two places.
Employee was cleaning bathroom and was stung by a bee. Taken to hospital on 7/21 and released on 7/21. Returned to work 7/24 (Monday).
Employee was drilling and scaling face, rock fell striking employee in the right shoulder and back causing bruising
Employee was in the process of tightening a pipe T with a pie wrench. The T slipped in vise and injuring the right index finger resulting in a fracture. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
A piece of wood off of a wood plank was blown into employees left eye by the fan on a bolter. Employee went to eye doctor the next day and received medication in the eye.
Struck by air hose above left eye.
Hot material fell onto jacket, then material fell down on top of employees right hand. (Submitting now, due to information obtained on 3/12/2018)
Contractor employee was loading tools into the back of the utility truck, EE threw a sledgehammer into the bed and felt something hit EE's eye. Cause of this injury was not wearing safety glasses while on duty. Compliance with company rules, PPE & miner attitude was a factor. Mining equip/systems not a factor.
I.E. was unlatching a 5th wheel trailer from the tractor ee was driving. The pin that locks the tongue in place was stuck so ee tried to loosen it by hitting it with a hammer. After it had freed up ee pulled the pin out, the spring pulled it back in which pinched thumb between the plate & tractor. I.E. had to have procedure done to fix thumbnail. No work was missed.
Miner was using a pick ax to dislodge a rock from a vibratory feeder. A portion of the tip of the pick broke off and became lodged in the miner's arm. All equipment was locked out and all safety equipment was used/worn.
