Accident type, without injuries

A roof fall was discovered in the #6 intake entry approximately 200 feet inby the main mine fan. The fall measured approximately 18 feet wide, 20 feet long, and 10 feet high.

A roof fall occurred in the 6th West Panel/3N/2ME between #4 and #5 entries at crosscut #21. The intake stopping in the crosscut was damaged. No injuries occurred.

IGNITION - Occurred on 6 section in the No. 1 entry. The continuous miner was cutting coal when a frictional ignition occurred. The flame was yellow-orange and lasted 2 to 5 seconds and went out on its own.

Operator running a machine noticed that there was oil coming out of a Trac-Hoe running in the area and before being able to warn the operator the machine caught fire. The fire burnt the wire to the shut off so the pumps kept running and feeding the fire. Operators walked away and called for the fire department. Operators done as they were trained to do. No one hurt!

An intersection fell in immediately after being mined through, in/by the section loading point. There were no injuries, or equipment damage, or entrapment.

An unintentional roof fall occurred in the #4 entry @ crosscut #360 on the MMU 001/002 section at spad #3684. The fall was approximately 9' high & 45' x 30' in the intersection. The area was supported with 5' resin bolts and 10' cable bolts. The material that fell was shale and absent of any sandstone.

A 12KV power line failure resulted in a 4.9 acre brush fire. A communication cable was damaged.

An electrical malfunction caused fuses to blow to the emergency hoist. Production was halted, personnel were withdrawn to the feeder line until repairs could be made. The back up power supply was initiated at 10:04am and the hoist returned to service. No persons were injured or ill during this incident

The mine experienced a roof fall in the East Mains Rooms Area, in the #3 Entry. (22ft inby Spad 8962). The fall was located during an airway examination. There were no injuries from this incident.

On August 28, 2017 at approximately 9:00 AM, a roof fall occurred in the Gateroad 404, CM Development Section. The fall occurred in the Last Open X-cut of Entry 2, at X-cut 68. The roof fell above the anchorage of the installed roof bolts. The CM operator observed the roof conditions deteriorating and had stopped work in the area and alerted others to stay away.

A fall of the roof occurred on the 2nd NE Mains (3rd North 54), #4 entry, crosscut 7, intersection. The fall measured approximately 20' length by 20' width by 7'-12' thickness.

An unintentional roof fall was found on 8/26/2017 at approximately 04:15pm. The fall was in the #3 entry at spad #5327. The approximate dimensions of the fall are 18' in width x 30' in length x 7.5' in thickness.

IGNITION - Occurred on No. 12 section in No. 3 face while continuous miner was cutting bottom on the right side of the face, the flame was extinguished with water from the wash down hose on the continuous miner.

Three employees were traveling in an elevator from the 2nd floor to the 1st floor of the preheat tower. The elevator traveled approximately 2ft and stopped due to a safety interlock fault code. The employees were removed from the elevator after approximately 45 minutes. There were no injuries.

An unplanned roof fall was discovered at 3:25 am in the 17-10 crosscut between the track and belt. The fall is approximately 26' wide 20' long and 12' high. No ventilation or travel ways were affected. The area has been posted and dangered off.

Smoke was discovered on the 4W end wall, in the upper coal seam. The smoke was not extinguished in 30 minutes. No miners or mine equipment were at risk during this event.

Unplanned fall discovered reportable at 11:50 am on 8/22/17 located in the first intersection outby the back up ventilation control for the #4 seal of the 3 North Bleeders Seal Set. The fall was approximately 7' high, 8' wide and 15' long. Area developed in 1965. Fall occurred due to gradual roof deterioration to the point of failure just above the 5' anchorage zone.

An unplanned roof fall was discovered at 10:45 am at the number 192 crosscut of the 5 North Left Return. The fall is approximately 16' wide 30' long and 12' high. No ventilation or travel ways were affected. All access to the area has been dangered off. The air way is common with another entry so the weekly examination has not been affected.

A roof fall occurred at break 29 on #4 belt. The estimated size of the fall is 20 ft. wide x 20 ft. long and 5 ft. high.

A record rainfall of 6.14"", the largest amount of rain in one day since the construction of the Stamper Mine portal, resulted in the mine's intake lake releasing water. Water traveled down the slope and into a localized area of the mine. We do not believe that this event is ""an unplanned inundation of a mine by a liquid"" and are submitting the 7000-1 at MSHA's direction.

Due to a noise in the bull wheel on the service hoist the hoist was taken out of service for repairs. Both agencies were notified and all employees were evacuated. No one was injured or trapped.

A roof fall was discovered near spad 600 and measured 35' in length 18.5' in width and approximately 7' in height. The fall was between #5 and 6 entry and did not fall into the adjacent intersection. No ventilation controls or escapeways were affected. No employees or equipment were involved. The area has been timbered off and all approaches flagged.

Smoke was discovered in the 4W coal fender near the bottom of the ramp. The smoke was not extinguished in 30 minutes. No miners or mine equipment were at risk during this event.

A Roof Fall occurred at 16 belt XC12 Beltline approximately 23' 9 "" from mine floor to roof. Screened and dangered off, Set Double row of timbers on all four access points, build cribs offside of belt from Roof Fall location inby within 25' of XC11. Ventilation was not affected, No injuries and no equipment involved.

This form is being submitted to abate citation 9075552. We experienced an overflow of refuse slurry material at the area #1 impoundment. There were no injuries or equipment damage. The flow was fully contained on site. Our investigation to date does not indicate that this meets part 50 reportable requirements.

SCI Materials's mechanic was preparing to jack up a stacking conveyor using a handheld 20 ton bottle jack. A vehicle was parked underneath the stacker. As the conveyor was jacked up it slowly tilted forward. The support beam of the stacker gently rested onto the hood of the vehicle, causing minimal damage to the hood and windshield. No injuries occurred.

A fall above the anchorage was found by the weekly examiner in the 4 South intake escapeway blocking the travelway at spad location 4+67. This fall was due to normal weathering of the area as the roof bolts broke off. The area was dangered off and the escapeway was rerouted. This area was mined in 12/07 with fully grouted 6' bolts.The fall is 50'Lx6'Hx18W.

While pulling ore from 5970 the north skip was accidentally double loaded. This caused the hoist to trip out due to the weight. All work was stopped and personnel were moved to secondary escapeways. Hoist was returned to service at approximately 15:30. 18 persons were UG at the time of the incident

Spray tower plugged which limited draft to the system, the cooler compartment fans were brought up by the operator and the pressure became positive in the kiln firing hood, which pushed hot air into the burner pipe causing fuel and the coal flex pipe to ignite. The fire dept was called to put out the fire.

A fall of the roof occurred on the #4 unit, 1st SW Sub-Mains, #0 entry, crosscut 18, intersection. The fall measured approximately 20' length, 20' width, 8'-14' height.

On 8/13/17 a power interruption occurred at the mine site causing a loss of power to the mine shaft/hoist. Mine management was notified at 6:14pm, management called the MSHA hotline at 6:19pm to notify MSHA of the loss of power to the shaft/hoist. At 6:22pm power was restored to the shaft/hoist.

Electricity from our power company had dropped out a couple of times throughout the morning. Each time all power was reset. At 10:30am the power dropped again, but the Koepe hoist took longer to reset/recover. Employees were evacuated from the mine via the production hoist. Koepe was operational at 12:30pm.

MSHA was testing the 3N6 escape capsule. It was lowered to the bottom and when it started to raise back up, the capsule got stuck between the guide cables. The capsule was out of service until they got it released from the cables.

A Roof Fall occurred in the intersection at 8 entry XC 11 the roof fall was approximately from 3-10 ft. tall above the mine roof horizon. The area had 8 ft. roof bolts and 12ft. cable bolts installed. All entries around the fall have two rows of timbers and are barricaded. Mine ventilation was not affected. No injuries and no equipment was involved.

An unplanned roof fall that was above anchorage zone of the primary support occurred in the #6 entry at 18 break on the #1 section. The dimensions of the fall are 30 feet long, 50 feet wide, and estimated to 8 feet high.

An unintentional roof fall was found on 8/9/2017 at approximately 6:50pm. The fall was in the #3 entry at spad #5290. The approximate dimensions of the fall are 18' in width x 25' in length x 6-7' in thickness.

After the coal shot was initiated, the dust cloud ignited into a large fireball & with gusting winds ignited native grasses/timber to the Southeast of the pitwall. The water truck & wild land fire truck were staged appropriately but the gusting winds carried hot embers from the fireball onto native grasses & the resulting fire spread very quickly.

A non injury roof fall was found at 7:00 AM on 8/8/17 at XC 4 entry 3 2rMS #1 belt. The fall was in a four way intersection that had been mined December 2014 and supported with 8' resin bolts. The fall will be cleaned up upon approval of clean-up plan.

Unplanned roof fall on the 12 Right Longwall tailgate. Top ""potted"" around roof bolts at the tail of the longwall face. Set additional roof props, tightened existing cribs and continue to mine through the area.

There was one employee trapped on Camp Run Elevator for approximately one hour and fifty minutes. The cause of this incident was found to be a gripper fault on the elevator.

The fire detection system alerted that a fire was detected in the burner building coal fines storage bin area. The shift supervisor and some crew members went to the area to determine if there was an actual fire. The supervisor determined that it was not safe to fight the fire and evacuated the area. The supervisor requested that the fire department is called.

The mine experienced a roof fall over the long-wall stage loader. The fall occurred in the #24 block intersection and measured approximately 12 feet wide by 20 feet in length by 6 feet in thickness.

Fall was outby survey station in the #2 neutral entry on Southwest Mains. It fell in an intersection and measured 30' long by 30' wide by 8' in thickness. It was located approximately 1800' outby the 010 mmu. No persons or equipment were affected by the fall. Set at least 2 rows of timbers on each side of the fall. Do not intend on cleaning the fall up. No ventilation was impaired.

The 20 person elevator at Williamsville intake shaft became inoperable due to wiring issue, all underground personnel were brought out of mine until car was back in service at 2:50 PM

The #2 Kiln Cooler Conveyor Belt was discovered to be on fire by the kiln operator. When discovered the belt was completely engulfed in flames.

At 2:45 pm management was notified that one employee had been stuck in the Blake's Ridge elevator for 30 minutes. The elevator car safety switch had tripped causing the incident. The fault was cleared and the employee was brought to the surface at 3:00 pm.

An unplanned roof fall below anchorage has occurred in #4 crosscut between the belt entry and the #1 Monkey Butt between 9M and 9N recovery faces.

A roof fall was found at 10:20 PM in a four way intersection of entry #4 SS 61+20 of the 2RMS (MMU 006). No injury or property damage occurred, travel-way was re-routed and access was propped off. Fall was aprox. 20'X10'X20' area had been mined June 27th 2017 and bolted with 8' full resin pins.

A non injury roof fall accident occurred @ approximately 2120 hours on 08/04/2017. The roof fall occurred on #3 section in the #5 heading in-by spad #8115 on the left side lift of the MRS which pulled a few of the Rib line bolts. This fall did not impede escape travel ways and did not effect ventilation.

An unplanned roof fall was discovered in the intersection of the #2 entry at 85 break on the old #10 belt area. The fall measured approximately 20'L x 20'W x 6 - 8' thick.

On 8/3/2017 at approximately 4:00A.M. a roof fall was discovered at 20 brk the No. 7 entry on 1 left 3 belt. No accidents as a result of this occurrence.

On 8/02/2017 a Roof Fall was found in the Main Return at the 1st North #2 Belt at 5x-cut. The fall was found in the #1 and #2 entries. The fall was called into the MSHA hot-line.

CAT D11R was pushing material towards wall. Operator noticed smoke coming from engine compartment. EE shut down engine, exited cab, observed source of smoke and noticed flames. EE attempted to extinguish with a hand held extinguisher and was unsuccessful. EE safely dismounted equipment to a safe location and notified EE's supervisor.

An unplanned roof fall was discovered in the intersection of the # 6 entry at 35 break on the old #8 Belt area. The fall measured approximately 20'L x 20'W x 6 - 8' Thick.

During normal operation of the hoisting system the deflection wheel broke free from its mounts and became displaced. This caused a misalignment of hoist head ropes.

A roof fall occurred in 67 HG LW section in the last open crosscut at 44 room 2 to 3 xc. No one was hurt or trapped. The crosscut was cribbed and dangered off on both sides. Ventilation to the face was not affected. The fall was 60' long and 8' to 12' high. There is a fault in the upper strata in this area that caused the fall.

The fall occurred in the #1 return entry of Southwest Mains 70 ft inby survey station 970. The fall was in an intersection and measured 20 ft wide by 20 ft long by 8 ft thick. The fall did not impair ventilation and we do not intend to clean up the fall. It was blocked off by setting 8 breaker timbers in all entrances. No persons or equipment involved. Roof fall only.

IGNITION - Occurred in the No.1 face of the No. 7 section. The continuous miner CM128 was cutting top when a frictional ignition occurred. The flame lasted approx. 45 seconds and was extinguished with two fire extinguishers.

An unintentional roof fall was found on 8/1/2017 at approximately 04:30am. The fall was in the conveyor entry at spad #5329. The approximate dimensions of the fall are 18' in width x 30' in length x 10' in thickness.

Fall onto or against objects

The employee was found unresponsive at approximately 2:05 PM on the offside of the #1 belt at the portal entrance of the surface mine. Preliminary findings has determined the employee received fatal injuries when coming in contact with the belt conveyor.

Struck by falling object

On August 3, 2017 a miner was fatally crushed while ee was dismantling a metal structure used to supply water to tech fine coal filter at the preparation plant.

Caught in, under or between a moving and a stationary object

EE was operating Roof Bolting Machine backing out of the #4 entry. EE had hand out of the operator's compartment when hand was caught between the rib and the roof bolting machine.

Caught in, under or between a moving and a stationary object

EE was changing out drill steels on a horizontal degas drill when EE inadvertently placed left hand between the pipe clamp and the drill steel. The hydraulic pipe clamp closed on the EE's left hand breaking the ring and middle fingers and amputating the tip of the middle finger.

Employee was removing guard from miner when guard slipped and caught end of employee's finger resulting in amputation of the end of middle finger.

Caught in, under or between running or meshing objects

EE was shoveling under conveyor which isn't common practice as cleanup is usual done after plant is shut down & bobcat is used. It was muddy & as EE stepped back felt was slipping & reached up to catch self & got fingers in between guard & pulley. Lost ring finger on rt hand between fingernail & middle finger, middle finger at middle knuckle, pointer finger just above hand knuckle.

Contact with hot objects or substances

Flushing material from a hopper made contact with operator's legs, neck, belly, mouth. This material was around 800 degrees Fahrenheit. Improper PPE.

Caught in, under or between a moving and a stationary object

Employee was working on the cone crusher. As employee was unscrewing the bowl, left hand slipped off the ram and as employee went to reach with right hand, right thumb was pinched.

Caught in, under or between running or meshing objects

Auger operator was handling auger steel and the middle finger of EE's left hand was caught between two pieces of steel, resulting in the miner losing one inch off the end of middle finger.

Caught in, under or between a moving and a stationary object

IE was working on a belt scraper when the wrench IE was using kicked back pinching IE's finger between the wrench and a piece of metal resulting in a laceration and fracture to the tip of IE's right ring finger. IE was seen at the emergency room where the decision was made to remove the tip of the finger.

Struck by falling object

A fully trained miner injured Rt. pinky finger when loading a ranging arm onto the longwall face a piece of rock fell from between the roof support striking miner's finger. The injury was not the result of any compliance issues nor were any protective guards involved.

Caught in, under or between a moving and a stationary object

Operating Mills Beater. The belt took employee's glove off and took off the tip of employee's finger.

Struck by falling object

Putting Screen up in Entry 2 when a rib rolled out causing injury to Hip Area. No equipment involved.

Fall onto or against objects

Was climbing into a skid loader when EE slipped on lime dust on the metal step plate. EE tried to catch self but right knee hit the metal step plate.

Over-exertion (Not Elsewhere Classified)

Employee was installing 60"" belt structure all day and wrist started hurting that evening at home. When ee woke up the next morning wrist was swollen and stiff. EE went to get it checked out and was informed ee had a fracture on left wrist.

Struck against a moving object

On 08/31/2017 employee was operating an end loader and ran over a rock jarring their lower back. Employee continued to work and on 10/25/2017 they went to the Doctor for treatment.

Caught in, under or between a moving and a stationary object

Injured was rib bolting in 4 East off the miner. When the steel was extracted from the hole, the steels wouldn't separate. EE put gloved left hand on the end of the steel and tried to extract with the hydraulic feed, but hit the wrong direction and pinched fifth phalange between the drill steel and the hole in the coal rib, resulting in a laceration requiring 16 sutures.

Struck by flying object

Miner cut a small diameter tree, which fell & bounced, causing the butt of the tree trunk to spring up, striking the miner in the chest. At the hospital it was determined, the miner suffered a fractured sternum, and that a laceration of the liver occurred, but had ceased bleeding. Miner admitted to Pikeville Medical Center.

The D-10R dozer was being disassembled in preparation to be moved. The push-arms that had been removed were sitting on the ground. Employee stepped over one of the push-arms and it fell, striking right leg causing a fracture.

EE was working on a 992 loader pushing a bearing in a lift jack and dropped a 40 ton ram jack approx 3 feet on left foot.

Employee was assisting bolting partner by helping self put glue and a bolt in the rib, as ee turned to walk away ee heard a snap. When the employee turned around ee was struck in the left hand by a broken wrench. This resulted in a fracture to employees left hand.

Over-exertion in lifting objects

Assisted coworker lifting hoses 8/30. Noticed L side pain next morning. Went to ER 9/1. Diagnosed w/abdominal wall strain. No treatment. Continued to work until 9/12 when off for unrelated elective surgery. Contact foreman 9/27 that ee had surgery 9/26 for hematoma thought to be caused by strain.

Fall from machine

EE was on a loader outside of the scale house. EE smelled a sewage smell and lost 3 point contact. EE fell to the ground from the top step onto 1x2"" gravel parking area. Took EE to the hospital for evaluation. EE has a sprained left elbow, hurt right wrist, and a few abrasions. No broken bones according to the x-rays. No MRI was done.

Employee was working on the slope belt approximately 700 feet from the surface when EE got left arm caught between the top belt and the top side and middle rollers.

Fall to the walkway or working surface

EE was removing block from a scoop bucket and went to walk away from the scoop and slipped off of wet and muddy cable and fell down. EE's left foot stayed stuck in the mud, when EE fell and twisted left knee.

Over-exertion in pulling or pushing objects

Employee was scraping out buildup in 28D-1 chute when felt a pop in shoulder. Employee seen by nurse. Employee returned back to work. Continued medical surveillance. Surgery to shoulder on 1-24-18.

Fall to the walkway or working surface

Employee slipped and fell from the slope due to high pressure water hose, loose rock, coal and debris resulting in EE fracturing lower right arm.

Fall from machine

The EE was operating the Hemtt fuel truck. It was raining and EE was running behind schedule because of the rain. EE was driving down a steep ramp to fuel a water jack and states that the brakes did not work. Before EE got to the bottom of the ramp EE decided to jump from a moving vehicle. Had EE stayed in the vehicle at the slow rate of speed EE may not of been injured.

Struck by falling object

While lifting a pump from sump to clean, the pump became unhooked from the chain hoist and fell on top of employee's left foot. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.

Struck against stationary object

The employee was moving a metal beam from one area to another. There were lifting chains on the ground nearby that the employee didn't see. While carrying the beam, the employee stepped on the chains, the chains and the employees foot slid out from under EE, and EE's knee twisted. Employee referred to doctor for evaluation.

Customer driving a 4 axle dump truck could not get tailgate closed after being loaded with gravel. Driver used hand instead of a hand-held tool to free a rock that was preventing latch from closing. Upon removal of the rock the latch closed on the driver's hand removing skin from the area between pointer finger and thumb.

Employee was walking down the long wall face when employee slipped and strained employee's lower back.

A fully trained miner was injured when putting a bolt in the roof. While using a bolt wrench to tighten the bolt the wrench broke striking lip. The accident was not the result of any compliance issues nor were any protective guards involved.

Struck by flying object

Employee hand pushing track rail off of flat car and when one end hit mine floor the other end bounced up hitting employee on left side of face in the cheek area cutting cheek (required stitches) also sinus fractures and a small hair line fracture in cheek bone

Over-exertion in pulling or pushing objects

Bottom of the dump, Muck area. Employee was cleaning the area when accumulations returned back down the belt and employee had water and muck up to the waist. When trying to remove themselves from the muck, they felt a strain in their lower back and right ankle.

Fall from machine

While unloading material from a low flatbed, employee was standing on low flat bed of truck, material shifted, employee fell off landing on ground. Fractured pelvis and hip, contusion throughout body from fall

Fall to the walkway or working surface

Injured was carrying a top roller (removing it from the walkway) just inby 59 block 1 East Head Gate belt (track side of belt line) and tripped, falling down and receiving multiple injuries. The most severe was a fracture to sternum and a fracture to the L1 vertebrate.

Struck against stationary object

Employee was dismounting from compactor, stepped on a rock twisting their knee. Doctor removed employee from work pending MRI results.

Over-exertion in lifting objects

Employee was carrying a box of resin glue to put on the roof bolter, when employee twisted to put the glue in the glue tray employee felt pain in employee's lower back and pain going down employee's left leg.

Over-exertion in wielding or throwing objects

While cleaning 1 North track haulage (around 13 block) with a mattock, employee hit one of the rails with the mattock and experienced pain in the lower back. Employee finished the shift, but went to the doctor afterwards and was given time off due to a strain of the lower back.

Fall from machine

Employee was standing near the engine compartment of a 928 Front End Loader when they felt the equipment move. The employee jumped off of the 928 Front End Loader and injured the left knee, right shoulder, and forearm.

Climbing down off the D10 to do a walk around inspection, EE missed the step. EE was holding on to the grab handle to catch self when EE fell and EE's body twisted, hurting EE's shoulder.

Miner was preparing to wash pick up truck with the water canon. Miner had placed the transmission in neutral and set the park brake. Miner went to rev up the throttle for the water canon when the truck slipped into gear causing the truck to lunge and the miner fell off.

Employee was pulling on the Continuous Miner trailing cable in order to get it out of the way of the continuous miner. EE strained left shoulder while doing so. *** Employee worked normal schedule until ee was taken of work on 12/13/2017.

EE was running a s/c. The s/c ran out of cable. EE parked the piece of equipment and went to see if anyone else needed help. During this process the EE was struck in the middle of EE's back, hip, and leg.

Ee was climbing a ladder to remove a trip cable at 351 shovel located at pipeline. Bango arm swung around and struck the ladder, causing the ee to fall approximately 5 feet to the ground. Ee landed on left side. Fractured left arm, bruised left knee, strained left ankle. Off work pending an appointment with orthopedic surgeon.

Employee was working in the plant area when employee exited the power building employee stepped down on a 2"" water hose and twisted employee's ankle. Employee had been using the hose earlier to wash out material around plant.

Employee was laying track through a mud hole in F28 HG. Employee felt pain to employee's lower legs from coming in contact with concrete mix used in the mud hole. Abrasions and burns to both lower legs.

Employee was removing a temperature sensor form the rear axle of a wheel loader. Employee tried to back the sensor out just enough to bleed out any accumulated pressure, but went too far and right hand was sprayed with hot oil. First and second degree burns to palm and wrist.

While pulling down a ratchet tightening 993k front wheel unit mounting bolts at Simmons Fork straining lower back. Employee did not seek medical attention until 10/24/17 and didn't send in paperwork until 11/1/17.

Employee sat an excavator tooth on a handle of water cannon. Employee moved handle causing tooth to fall on top of head. Employee had removed hard hat for unknown reasons. Employee received a cut that required a couple of stitches.

Employee states walking over to left return brattice line just outby # 2 unit to get a flashing light for the unit. Stepped on a rock with left foot and while stepping over with right foot the rock shifted causing them to fall putting all their weight on left toes causing fracture to small toe at base of the foot.

EE will be off work recovering until 12/7/17.

EE hooked a chain to one side of a plate and the other end was connected to the excavator. The operator lifted the boom to flip the plate, EE pushed the plate with EE's hands causing slack in the chain, which slipped off the plate and landed on EE's feet.

This employee was dragging a shuttle car cable around the block when EE caught EE's foot on a stone and started to fall. When EE tried to catch self EE jammed EE's leg into the ground, which jammed EE's leg into EE's hip area. EE also states that EE landed on EE's Proximity box that was on EE's belt. This fall caused 2 fractures to EE's left hip.

Two weeks later the employee started to have swelling in the ankle and was looked at again and it found that employee had an abscess in the lower leg and had to have minor surgery to remove infected area.

During this process the flask broke and a piece of glass penetrated a nitrile glove and cut the palm of the injured employee's right hand. The injury resulted in the employee receiving 6ea. stitches and a return to work release with no work restrictions.

EE was tramming the scoop, the bucket of the scoop hooked the pump cable that was hung damaging the cable. EE got out of scoop to hang the cable and when EE grabbed the cable EE got an electrical shock.

The employee has not missed any scheduled shifts until after surgery.

The cable was hung in D-rings and the hooks straightened out, letting the cable fall and striking the injured in the head and both forearms. Contusions to right forearm was the most severe injury.

Transmission for CAT 16G motor grader was being installed while on crane. Employee was pushing on transmission to align it when they felt a pain in their back.

EE began missing work as of 08/12/2017.

Employee was pulling a supply trip around the loop. The trolley pole came off the wire in a low spot. As the employee reached for the pole, apparently touched the metal portion of the harp & received an electrical shock. Employee completed shift. Experienced discomfort in arms later in the day and visited doctor. Employee states has difficulty raising arms.

EE attempted to step over a cable that was left in the walkway when their boot caught the cable and tripped them causing them to over extend the right calf and Achilles tendon. As of the date of this report, BUU is questioning if this is a pre-existing condition. MRI is being scheduled; off work 9/3/17.

Employee was walking backwards dragging a spray pump. Employee tripped over a drain line attached to water tank and fell backwards dropping spray pump on lower right leg resulting in a fracture of the fibula.

Employee was grabbing a sheet of wire screen from a partial bundle on the rib, started to turn with the screen when the rest started to fall causing the employee to fall to the ground. Employee was removing an excavator tooth shank from the excavator bucket. Employee air arced existing welds and EE struck the shank with a 16 lb sledge hammer. The hardened steel sledge caused a piece of hard facing to chip off of shank and embedded in right thigh. Had foreign body removed on 8/18/17 with stitches - 1 day lost time.

Operator was climbing into excavator. Hand slipped and lost grip, fell backwards onto their back on the ground. Has previous back surgery, now has low back pain. We called their surgeon who said go to Emergency department.

While repairing the daybox on the emulsion truck, employee was under truck with open end wrench on bolt holding tight while fellow employee was on topside using an impact wrench to loosen a bracket. While cleaning a belt drive with a rake, turned and struck left elbow against a steel I-beam. EE continued working and finished the shift. Diagnosed as a bone chip type fracture along with a ruptured bursa sack in left elbow. Limited to a ""No use of left upper extremity"" restriction.

On 19 August at approximately 11:00 AM an underground mechanic sustained an injury to abdomen when a suspended load fell and contacted EE. The employee was in process of lifting a Kubota tractor axle with a crane when the load fell and struck EE. The load fell approximately 18 inches and weighed 150 lbs.

Pipe was plugged, employee took bolts out of flange and moved one pipe up so employee could look into the pipe to see where the blockage was. As employee held the flashlight in the pipe, the pipe settled pinching and breaking employee's pinkie finger, left hand. The finger is broken.

A reactor floor operator was using an air lance to unplug a line. When the plug of clay in the line broke loose, the air lance became stuck. The operator gave it a tug to free it and felt a pull in groin area. A hernia was diagnosed by the physician.

At approximately 1950, a crusher trainee was preparing the #2 shorthead for maintenance by pulling back the dust curtain. While doing so, they tripped backwards and fell hitting the handrail. Their right arm was caught up in the rail and twisted the arm.

Employee was burning wood debris in the main pit area. Employee was too close to the flames, resulting in 1st and 2nd degree burns to face and arms.

On 8/16/17, the employee received medical treatment for a right wrist fracture, was placed on restricted duty which was accommodated and subsequently lost time on 8/28/17. The employee's pant was caught on a fence post that was leaning against a truck while walking backward carrying a fence with a co-worker. This caused EE to fall backward; EE used arm to break the fall.

The injured employee was installing a fill valve assembly on a product tank. As the employee was adjusting the valve with a 24"" pipe wrench it slipped off the valve assembly causing employee to fall forward into the side of the tank. As employee fell forward felt a pop in right shoulder.

EE hand scaling and felt pain in low back and right shoulder. EE placed on restricted duty following medical examination.

Over-exertion in lifting objects

Employee received a laceration on ee right index finger when it was smashed while lifting a heavy object on the 9L Plow Head Gate.

Struck by falling object

The site was lowering vanes from a coal mill via a pulley and rope system. A vane fell against the leg of the employee injuring EE's left knee.

While attempting to line up two pinch tube flanges the spud wrench that was being used slipped and struck the employee above the left eye. The resulting injury was a laceration that required stitches.

While preparing to change out a grease hose reel on a fuel/lube truck, the reel fell striking a lubricating technician's right hand pinky finger.

While removing rock from canopy, finger got pinched between rock and canopy.

Employee was exiting the mine when they struck their face/nose on the frame of a manual airlock door.

EE was struck in the forearm with a metal fragment from the tip of a grader that became dislodged when another associate was removing the end with a 3 pound hammer. The incident occurred on 8/31/17 & was first aid only. After a few months, the embedded object became infected & resulted in a medical procedure to remove the object at the doctor's office on 12/8/17.

The employee's hand was pinched between the 6"" HDPE pipe and the middle post on the work deck when the pipe slipped off the end post. The contact caused a laceration to the palm of the right hand.

Dozer drove over a rock and jarred employees back. EE did not report injury and sought chiropractic care the next morning. Injury was reported to management on Saturday the Sept 2nd. We are unsure if chiropractic care counts as a recordable so we are reporting this just to be safe.

EE was mounting a cutter motor on a continuous miner. EE lost balance while stepping off on an 18"" high rail causing EE to fall backward. A pry bar EE was using struck EE in the right side as EE fell causing a 1"" laceration.

While cutting a piece of plastic with a hole saw the circular cutting bit jumped and struck the employee on the left wrist resulting in a laceration. The resulting laceration required stitches.

EE was raking ash when a rock hit the opposite end of the rake causing the rake handle to jump out of the EE's hand and strike the EE on the right hand thus causing a contusion to right ring finger.

Team Member felt pain in right elbow while attempting to start weed eater. This was initially reported as first aid. On November 13, 2017, ee was given prescription medication for the continuing pain in elbow making this injury reportable.

Normal working section - employee pulled a strap from the back of the bolter and cut arm. Employee did receive 7 stitches. *NO WORK MISSED

Employee helped pull a suction hose that was stuck in the mud out of the Phase 1 Hydro tank. Employee reported that EE felt pain in EE's right shoulder. On February 14, 2018, employee notified the employer that EE had started physical therapy for EE's shoulder.

Employee was removing a carrier plate off of a D11 dozer and had their left thumb pinched between the plate and the swing frame. The employee pulled their thumb free and noticed it was bleeding. Local medical center doctor advised employee to come in after the shift. The wound took 2 stitches to close.

EE was hitting a bearing with a sledge hammer and a shard of metal came off and entered right forearm.

While pulling loose trolley wire down track with vehicle, trolley wire got caught on something and when employee stopped to free the wire it broke loose and sprung forward hitting employee in the mouth. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.

An employee operating a 992 Cat Loader was trying to put a vent cover that had falling off of the AC unit during operations. As EE was trying to snap the cover back into position the cover slipped and the momentum the employee was using caused EE's hand to go inside the vent resulting in EE's hand coming in contact with the fan cutting EE's index and middle fingers.

WHILE PREPARING TO MOVE A QUARRY SAW, MINER WAS TRYING TO DISCONNECT THE MAIN PLUG FROM THE SAW BY PULLING DOWN. EE STATED THE PLUG SEEMED STUCK SO EE PULLED HARDER. WHEN THE PLUG FINALLY RELEASED EE'S HAND STRUCK A ""J HOOK"" WE USE TO SUPPORT THE SAW CABLE CAUSING THE CUT ON THE TOP OF EE'S HAND. TAKEN TO CLINIC FOR EVALUATION. FIVE STITCHES RECEIVED.

Riding to section in a Mac 12 rubber tire mantrip. While making a turn toward section the Mac 12 hit rib catching SCSR storage box shoving it onto a fire extinguisher and the fire extinguisher hit employee's hand. The right hand palm was cut requiring 6 stitches

Contract employee was helping another contractor to change a roll of felt material, cardboard tube got caught on roller and while using a knife, contract employee accidentally cut finger. Cause of this injury was not paying attention to the task being performed. Compliance with company policies a factor and miner proficiency, mining equip/systems, PPE, not a factor.

Miner was guiding saw track into place (track was suspended <1 foot off ground by crane)and had thumb between two track sections; the section being set and its partner track that was already set. Two pieces came together and thumb was caught between. Sent to clinic for evaluation. Three stitches received for cut. No Rx, no missed work days.

Fall to the walkway or working surface

Miner was walking down the rock ramp that had been dug out at the surge pile to reach the feeder. The miner was part of a crew working on the surge pile feeder. It had been raining all morning and the rock was wet. The employee slipped and cut a finger on the rock as they caught themselves.

Employee was using a ratchet to tighten a bolt on the shearer spray arm. The ratchet slipped off of the bolt head causing ee to strike knuckles on the frame of the machine. This caused a laceration on right ring finger that 3 stitches was used to close. No lost work days

Employee was at the slope, a rock 6 inches in diameter rolled down the slope, the employee blocked it with their hand

Employee was replacing a dredge spud cable and dislocated right thumb while using a pry bar. This injury was reported as the doctor put a splint on thumb.

Employee was chipping hardened soil from the flange of a stacker inlet so that the flange could be changed. Employee was using a spud bar in a standing position above the inlet. On downward stroke spud bar hit rubber belting and bounced back up causing a small laceration above employees right eye. Employee was taken to medical clinic and received 7 sutures to close the laceration.

Employee reached behind a tool box to retrieve some welding rods. As employee pulled the welding rods out the employee lacerated left forearm on a sharp edge of the toolbox. Employee received 10 sutures to the wound.

Employee claims to have been struck in the mouth by a rock while hanging curtain that may have resulted in a broken tooth or teeth. Medical evidence suggests that the cause and extent of the alleged injury does not support that it is a reportable injury. This submittal is made as precaution.
