An unplanned roof fall was discovered at # 9 block of the # 2 entry on # 27 butt section track entry. There was no equipment involved, and no injuries, and the ventilation system was not disrupted. This occurrence was immediately reported.

The #4 entry on the longwall face on 17 east sat down. Notification made and issued a K order on the section due to the roof.
The #9 Portal elevator went down with no people on it at 12:00 am. The elevator was put back into service after the door limit switch was replaced at 4:15 am of the same day.
At the Leeville mine in 4000 Level North a rib that was in the process of being re-supported failed. This temporarily trapped a contract employee with Small Mine Development (SMD) in a mucker underground. The employee was successfully rescued at 6:08 am, uninjured.
Ongoing rehab in the 4000 LV. Operator was peeling lt. rib and was making operator's last cleanup pass to bring in the bolter. Rocks began hitting operator's cab and then the back came in on operator. This area is known to be faulty ground. Operator was able to make visual contact with coworkers.
A roof fall occurred on the 2 East Long Wall tailgate that blocked the passage for miners to travel the tail gate side which is 1 East #1 Entry.
The service hoist at the Camp Creek Portal went down at 11:37 am due to a bad power supply for the surface I/O rack. The power supply was repaired and the hoist was put back in service at 12:16pm on 06/26/2017.
On June 25th, the hoist went down around 9:00pm. The electrician on shift was not able to diagnose the problem so the hoist remained out of service for the remainder of the shift. The day shift electricians found that the hoist drive had tripped on over-voltage. They adjusted settings to reduce the secondary voltage and brought the hoist back on line.
Employee was using the crane on the service hoist to pick up a shuttle car that was being transported outside when the crane on the service hoist went down. The actual service hoist was operational. No one was trapped or injured.
There was an electrical short in the inspection control switch located on the control panel inside the Miracle Run Elevator. This caused the elevator to be taken out of service.
#9 Portal elevator went down at 8:11 pm with no one on it. The elevator was running again at 9:56 pm after repairing a governor rope fault.
Driving up the lime dump road the operator of the articulated truck noticed an orange glow coming from the right back corner of the truck cab. The driver then got out, discharged EE's fire extinguisher, and called on the radio for help. Two other employees brought up 5 fire extinguishers, emptied them, and called the fire department. The fire department extinguished the fire.
Coal shot was detonated in Pit 2 at 2:30 P.M., shortly after smoke was seen coming from area south of Pit 2 beyond topsoil strip area. CCFD was notified and responded to the fire with mine personnel. Fire was extinguished after 30 minutes. No mine personnel, equipment or facilities were at risk at any time.
An unintentional roof fall occurred while retreat mining was in progress on the # 3 left section in the # 4 entry at the # 32 block.
Hoist tripped out on main bearing temperature, cooling water line was plugged , electricians flushed the line reset and hoist was up and running by 0045 hrs June 22nd
During regular inspection of the Hoist Car, a bad wheel bearing was found. The Crabapple Hoist car was taken out of service to repair.
An unintentional roof fall occurred in F28 TG Miner Section in the #1 entry just inby 25 wall from spad #62+21 to 62+53. The fall measured approx. 12ft high by 32ft long by 12ft wide. There was no injuries as a result of this incident. Conditions contributing to the roof fall include adverse geological conditions in this area.
An unplanned roof fall that was above the anchorage zone of the primary roof support occurred in the number 10 entry on the number 1 section of the American eagle mine. 70Lx20Wx8H. There were no injuries associated with the fall.
The magnet for the North Production Skip Gate Switch malfunctioned. The mine was safely evacuated while troubleshooting was on going. The magnet was replaced at 2:00am on 6/20/2017.
Entrapment of 2 persons on Wana Elevator for approximately 45 minutes. The cause was the Lilly was out of calibration.
Employee found the roof fall on the #2 section in the #8 entry at 25 block in the LOB. The fall was found at 9:00AM. The specific location was 1 block inby spad 2782. The measurements are as follows: Length was 28 feet, width was 23 feet, and height was 12-15 feet. There was no injuries that occurred.
No. 3 skip stalled above 5200 level, ""E"" brake set itself, when hoist man reset the limit switch would not reset. Electricians corrected.
The #6 portal elevator went down at 9:15 am with no one on it. The elevator was running at 10:00 am after the door switch was repaired.
Damage to skip shaft hoist equipment. Broken latch and bent guide rail. Expected to be down 4-8 hours.
Fire extinguisher on pickup truck did not have a visible annual inspection conducted. No damage or impairment resulted at this time.
After receiving the results of a non-destructive rope test, in which, the tail rope on the service cage exceeded retirement criteria, the rope had strength loss of greater than 10%, the #1 shaft service hoist was taken out of service until new rope is installed.
Mine lost power 6/11/17 at 4:45 AM & all personnel were evacuated. On 6/12/17 an electrician entered to check pump with gas detector (natural airflow of 55K CFM). 1280 ft. down the decline the EE encountered a rib fall on left side with material impeding travel, returned to sur., notified mngt. at 9:45 AM. During msha investigation est. 50-75 tons blocking 25% of decline
A fire occurred on Unit # 30305 while driving down the road to the drill bench.
An unintentional roof fall was found on 6/9/2017 at approximately 11:30am. The fall was in the high voltage entry at break 224 approximately 1 break outby spad 5357. The approximate dimensions of the fall are 20'X 20'X 7'
The BMX Portal elevator hoist was out of service due to normal wear to the linkage on the inside door causing the doors to malfunction. No persons were injured or entrapped due to this event.
Shredded plastic used for alternate process fuel that was stored in a covered containment caught fire. Unknown if the cover or plastic ignited first, cover was burnt off the structure and surface of plastic's was burnt. Source of ignition unknown. Fire was extinguished but had exceeded 30 minute time frame, call was made to MSHA hotline. Report to be filed with area MSHA office.
A rock fall at cross cut 11 on 4 Mains roadway in the intersection. The fall was 8 ft to 11 ft thick. It was supported with 5 ft resin bolts and 8 ft rope bolts.
Smoke was observed coming from the raw coal pile at approx. 6pm on 6/7/17. A track-hoe was dispatched to the spot observed & began digging our the spot where the smoke was observed. At approx. 8:45am MSHA inspector arrived on site and questioned if we had reported the incident. Part 50 is a fire not extinguished within 30 min not smoke! We were issued a citation.
Roof fall occurred in the 3RWL longwall bleeder in 3 entry @ 7 x-cut. It is uncertain when the fall occurred as it was 150' behind LW face and no person witnessed the fall. The fall measured approximately 21'x 60'x 14'high. The area out-by was supported with double-row of wood cribs. MPLs for weekly exams of the bleeder have been set and approved by MSHA at x-cut #6 & 11.
IGNITION. The roofbolter #140 was being used in #2 face. The operator was spinning the roof bolt through the hole in roof strap when an ignition occurred. It lasted about 30 seconds and was extinguished with rock dust from the roofbolter dust box. MSHA inspector, took air reading at end of blowing curtain 10,080 CFM and CH4-0.
At approximately 8:40am a contractor employee was using the passenger elevator in the cement silos. The elevator experienced a problem that shut the elevator down while the elevator car was positioned near the 4th floor. The employee was inside the elevator car for approximately 38 minutes.
At approximately midnight on 6/4/2017 several small fires were noticed in the new alternative fuels system. Personnel extinguished the fire using fire extinguishers and called GFD as a precaution. Upon further investigation, it was determined that a dust ignition/explosion had occurred inside the system's drag chains causing deflection damage to multiple covers.
Two employees were stuck in the #1 Crusher elevator at 12:40pm on 6-04-17. The elevator was stuck between the basement floor and the 1st floor. After an electrician could not get the elevator working again they had to wait for the elevator company to arrive out of Duluth to get the elevator to the basement floor. This occurred at 2:25PM.
An unplanned roof fall occurred on the 1st Southwest Mains in the belt entry at crosscut #28.
An unintentional roof fall was found on 6/3/2017 at approximately 5:02pm. The fall was in the belt entry on #10 belt between 226xc and 227xc approximately 15' inby spad # 5360. The fall was approximately 60' in length and approximately 4' to 8' thick. Timbers were set and both approaches to the fall were dangered off. No injuries occurred as a result to this accident.
The employee noted seeing a 'fire ball' come up from the bottom of the system. The employee jumped from the forklift and attempted to fight the fire with a hand held fire extinguisher and eventually called crew for assistance.
An unplanned roof was discovered in the #7 Entry on the #1 Section at approximately 8:50 a.m. The roof fall measured approximately 90 feet in length and was approximately 6 to 6 1/2 feet thick. It occurred approximately 25 feet outby the #23 crosscut to approximately 40 feet inby the #21 crosscut.
EE became stuck in the man elevator of the Fine Crushing Plant. A mayday was called along with electricians. Electricians were able to get employee out of elevator. Elevator locked out and elevator company will troubleshoot and correct issues.
Employee was bolting in the 4340-136 when the left spring line started to ravel causing material to fall above bolt anchorage. The estimated fall was 20' wide x 12-15 deep x 10' long. There was no injury to the event.
While performing the annual alimak over speed break test the lower limit switch tripped out causing the alimak car to settle in the pit.
Injured employee sustained fatal injuries resulting from head trauma after jumping or being thrown from the lead locomotive of a supply trip at crosscut 16 on east mains track.
On 6/13/2017 a continuous miner operator was moving the continuous miner out of the #3 entry on the #1 Section. As EE backed the miner out of the entry EE was caught between the head of the continuous miner and the coal rib. This resulted in fatal crushing injuries.
Employee was maneuvering around under a haul truck during an inspection and twisted/moved just right to exacerbate a pre-existing hernia condition. (Employee planned on having the hernia operated on in November 2017). EE finished shift and worked 2 hours, went to doctor after.
EE was at crusher console adding hyd. oil to reservoir. While shutting off valve at nozzle end, the weight of hyd. hose abruptly pulled out slack in between hands & into chain holding console door in open position. The weight of hose on chain caused door to start coming downward. EE tried to pull hose clear of lid when lid caught the EE's right hand in between it & console frame.
Employee was rotating a sheave block to uncross the two cable lines. When the sheave was rotated the weight of the cable caused the sheave to roll a couple of inches. EE right hand was pulled into the sheave as ee was holding onto the cable. Result was the amputation of the tip of ring finger, right hand
WORKING ON BELLY PAN ON DOZER. BELLY PAN FELL OFF THE JACK AND SMASHED FINGER ON RIGHT HAND
Replacing screen on Horizontal screen deck had climbed inside to position a screen when EE felt a discomfort in left shoulder. That evening the pain became intolerable so EE went to a medical facility.
Installing cement mill plates. Half of the line was installed and the cement mill was rotated in order to install the rest of the line. When the mill was rotated some plates fell, the contractor employees entered the mill for scaffolding installation. While installing the scaffolding, the plates fell impacting a contractor causing right ankle and rib fractures.
Employee was riding in a manbus traveling from the section to the bottom and struck knee against a pump on the left hand side of the track. Employee had laceration to left knee that required sutures.
Mechanic was trying to free the end of the hard bar that had dropped down and wedged into the roller frame. They were using a hyd. jack to push the roller frame out placing crib block under the bar when it came free from it's position as EE was placing a crib block under it and the roller frame shifted inward striking mechanic on the head on EE's hard hat causing a cervical strain
Employee was dragging a ventilation tube when ee felt a strain in hand. EE followed scheduled work schedule until seeing the doctor on 7/12 at which time ee was taken off work. First day missed 7/12
Employee was removing rock from a jam. Employee got right hand smashed between a rock and steel bar. Employee received a laceration requiring stitches.
A fully trained miner strained their right wrist while picking up rocks and loading onto the belt. The injury was not a result of any compliance issues nor were any protective guards involved.
NOTE: This incident was not classified as reportable until the date of July 22, at which that time employee began to miss work. Description: Employee was tramming shuttle car towards the miner and the tire ran up and over a corner ledge which caused employee to bounce up into the canopy.
While working to unplug a feeder/draw-hole at the primary surge pile, the employee became entrapped when material sloughed from the surge pile. Employee became dehydrated while entrapped.
Haul Truck down shifted after hitting bump while driver was reaching for their lunch box causing employee to feel stinger in lower back.

Employee was cutting bolt holes with a utility knife, when changing a rubber curtain underneath a crusher. The knife slipped off & punctured employee's leg.
Employee was helping install roof bolt with a gopher drill at 7 1/4xc in 4WHG when a rock fell striking left hand causing a sprain and laceration. Employee did not miss work until 7/7/17.Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was bolting the 2nd cut out of 4R. While installing a rib bolt in 3rd row of bolts a piece of draw rock measuring 70.5"" long, 0"" to 6.5"" thick and 46.5"" wide fell out from the top and the edge of the rock caught the back of EE's right heel causing an avulsion. **Also fracture to heal bone.**
The employee was entrapped and sustained injuries while acting as a first responder to 2 fellow workers who had become entrapped at the primary surge pile.
Employee strained employee's right side while carrying an old sprocket shaft from the Prep. Plant to a trash bin. Employee did not violate any rules, regulations, or policy and has proper training. Employee was wearing proper PPE and has a good attitude for safety.
Employee was building a block wall on top of scaffolding. While in a kneeling position the employee lifted a block to hand to another employee. When handing the block over to the other employee they felt pain down the lower R side of their back and leg.
EE was working at the construction area of the East Portal. EE started a diesel pump and part of the hose coupling came loose causing water to strike employee on the arm. EE received a strain/tear to the tendon on left arm.
Injured was walking in #3 entry of K section from Ram-Car to Continuous Miner to help with cable handling during place move. The Pavement in the area was covered in water and the injured stepped in a hole filled with water and twisted Right Knee injuring meniscus in knee.
Employee used a 10 foot ladder to climb the bowl mill structure to inspect the top of the bowl mill. As employee was climbing down, accidentally placed left hand near an unguarded drive belt and pulley located approximately 15ft off the ground. Employee received a serious injury to left pinky.
Climbing up steps of hoe, put foot on track & slipped & fell forward. Right side rib hit step of catwalk.
Employee was installing a 6' bolt on the 4E longwall face under the mesh when a rock rolled out from the binder area striking employee's right middle finger resulting in a laceration. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
employee was not wearing gloves and ring wedding band on ring finger caught or snagged on conveyor belt frame as they hopped down to the ground about 2 ft causing partial avulsion.
On 6/28/2017 employee reported to the supervisor that EE had hurt neck while operating a 4 yard loader on 6/27/2017. EE stated that EE hit a bump in the road and jarred EE's neck but thought EE would be okay. EE was seen by site medic, referred to ER, X-Rays were negative, restrictions were issued for a strain.
Employee was removing old rollers from 3 North #6 belt. Employee bent over to pick up a roller and felt pain in the right hip and back. Diagnosed with a herniated disc on 9/20/17.
On the above date, time and location, Security Officer was working the main gate of the Four Corners Mine when EE's chair hit an indentation in the floor causing EE to fall from the chair. The arm of the chair hit EE in the chest. EE did not report the incident at the time of the incident.
Employee was carrying a replacement screen cloth up a conveyor catwalk when ee hit a support with the cloth injuring right shoulder. Injury was a dislocated shoulder.
The worker was walking down a slight incline on a hillside while inspecting a fenceline at an area away from all mining activity. While in stride, employee's lead foot (right) slipped on dry grass. As momentum carried employee forward, back foot caught on something and stayed in place causing a sprained left ankle.
The employee slipped while walking underground and twisted left knee. Began missing work on 9/14/2017.
Employee was helping recover belt structure. EE bent over to pick up a piece of structure and felt pain in the back.
Crew was pressing dies into place on jaw using a portapower. Portapower unit broke, which caused the die to swing out and roll employee's foot, which caused the steel in the steel-toe boot to pinch foot.
Employee states while working on clean coal vibrator, after finishing, back allegedly started tightening up.
Employee stated that while carrying a longwall cable handler through the headgate area, EE stepped on a board and felt a ""pop"" in the left big toe. EE was diagnosed with a strained flexor tendon. EE didn't violate any rules, policy, or regulations; was wearing proper PPE & adequately trained.
Removing jammed rock; pinched finger between bar and bin wall.
Employee was aligning cells and stepped back and twisted employee's knee. Employee was taken to the ER and referred to an orthopedic doctor. Currently Ongoing.
Employee was diagnosed with an umbilical hernia by EE's doctor. Employee did not report incident at work but later communicated that EE believed it occurred while actuating gates under a rail car. It is probable that hernia was created by the repetitive stresses associated with opening gates under the rail car.
Injured employee stated, ee and another employee were prepping to change a drill pot on a bolter. They attempted to lift and load the new pot for transport to the bolter. Injured employee felt a burning sensation in abdomen during the lift. Employee was forwarded to a specialist by the mine nurse. The specialist saw and diagnosed the employee with a hernia on 07/19/2017.
Injured was walking back from 4E section power car after putting power back on the section auxiliary fan and attempted to step over some ventilation tubes. EE tripped and fell, landing on right elbow. EE continued to work until 7/1/17, when EE went off work pending surgery to repair a torn tricep tendon in right elbow.
Employee was bending over to pick up gloves and threw out employee's back.
Employee was cutting belt when knife slipped off of belt causing EE to cut left wrist. This did not involve any compliance issues. Personal Protective equipment was being used and this did not involve job proficiency attitude, or machinery.
Employee states while removing belt framing from 1B belt drive felt pain in lower left side of back.
An employee reported on 6/26/2017 felt pain while getting out of car, EE stated it was not work-related. On 7/12/2017 EE stated that EE had been in a squatting position cleaning a grating floor on 6/24/2017 and felt back pain when EE stood up. Injury had no lost work days until Nov 2018.
A fully trained miner was installing a rib bolt when *a rock fell from roof striking left shoulder*. The injury was not a result of any compliance issues nor were any protective guards involved.
Employee claims while performing maintenance task hit with hammer on index finger of right hand.
Employee was cleaning at end of shift. EE was sweeping up Hydrated lime with a squeegee and a dust bin when lime from the pile became airborne due to the dust bin adjusting quickly and a small amount of dust got into the employee's eyes. The employee did not report any injury until Sunday 06/25/2017 @ 10:50 am. Employee was instructed to visit a clinic and then file a report.
Digging out chute. Employee has been working regular job since June, was treating with therapy. Employee has not improved and is going for surgery Fri. 9-15-17. Employee will be off work following surgery.
EE was walking up the slope belt to change an amp box, when they slipped and twisted their ankle. They continued up the slope, completing the task and walked back down the slope.
Employee stated they had stopped the Mac-12 Mantrip near a scoop in the #4 entry to attempt to pull a cable using the mantrip when the mantrip rolled into the scoop pinching their left leg. This injury resulted in a fractured left femur.
Closed 7/3-4 for holiday. Injury reported to mgmt on 06/29/2017. Left knee pain from lifting Telsmith Jack(~70lb) on 6/23. IE sent to USH to be treated. IE immediately released to light duty. Restrictions of NO: lifting >10lb, climbing stairs/ladders, squatting/kneeling, driving mobile equipment; limit walking 15min/hr in 8hr day. Due to nature of job, no light duty work available.
Employee was setting up roof bolter to bolt last row of bolts, jerked down a small coal brow with T-Bar. It landed on top of the canopy then slid off striking employee's right thumb.
Employee was inspecting a dozer when EE slipped on the dozer track. EE caught self via the handrail and experienced pain in right shoulder. EE was evaluated by a physician and released without restriction on 06/23/2017. On 12/22/2017 a rotator cuff tear was found and repaired surgically.
Injured was building a brattice with mandoor in it. The employee was on their knees cutting a solid block that would be used to tie in the outby end of the stopping to the mine roof when the wall fell. Some of the block fell on the employee smashing right 5th finger (little finger) causing lacerations and fractures.
A fully trained miner injured left shoulder while scaling the roof. A piece of rock fell while EE was scaling and struck EE's left shoulder. The injury was not a result of any compliance issues nor were any protective guards involved.
While going thru a man door, Employee's foot got caught and employee fell to the ground. Employee complained of back pain that radiated down into leg. Employee worked all scheduled shifts until employee aggravated it on 10-12-17. After several doctors opinions employee's injuries were considered related to the 6/22 claim and became a lost time on 10/27/2017
EE was helping 2 other employees lift a shuttle car speed reducer on to a 4-wheeler when EE felt something pop in right knee. EE works a 4 on 4 off schedule and was scheduled to return to work on 6/26/2017 in which EE's doctor stated was unable to return to work.
Employee was washing down the 3rd floor of the prep plant with a wash down hose. Employee stepped on a slick spot causing employee to strain right knee. ***Employee did not start missing work until 11/15/2017. ***The WC Claim was deemed compensable on 11/30/2017.
Miner was building a return wall stopping when rib rolled out catching miners left hand between block and rib roll.
Employee was stepping off the D8D7 dozer and as employee reached the ground with left foot first, employee rotated to the left and felt a pop in left knee. The ground was just graded flat.
Employee was pulling transmission in a mantrip when the torque converter slid out. After reaching down and sliding the torque converter back onto the transmission, EE straightened up and felt tension or pressure on lower back. The employee continued to work until 06/04/2018, at which time the doctor took EE off work.
Rigging a bundle of rebar with chains, bundle was raised, a 2x4 was placed under the bundle. As the bundle was lowered, the employees' pinky finger on right hand was caught between the 2x4 and the ground. Produced an open fracture of tuft of distal phalanx of finger.
EE was walking back to loader which was parked near a 5ft ledge. As EE was climbing in EE placed hand on the loader to get in, slipped and lost balance. EE fell backwards down the ledge.
While shoveling at #2 drive felt burning sensation in lower back
Employee had started walking down H-1 longwall face and when EE went to step over the leg jack on shield #6 EE slipped and rolled right ankle.
While breaking a nut loose with a breaker bar and socket the nut broke free and foot rolled under body causing a fracture to the right ankle.
A fully trained miner injured their right hand(finger)when carrying a crib block and fell against the miner causing a laceration to their ring finger. The accident was not a result of any compliance issues nor were any protective guards involved.
Employee stated was operating the scoop on the #2 Section in the #3 Entry when the scoop went over an irregularity in the bottom, causing EE to be jarred in the operator's compartment of the scoop. This resulted in a left shoulder and neck strain and lost work days.
Employee, as a first responder, was assisting in transferring a personal illness patient from one ambulance to another, and injured back.
The operator of DZ 28 backed over a rock that was 1 foot above the grade of the floor. The unsuspected movement of the dozer aggravated a pre-existing medical condition in the employee's back.
Employee felt a pain in EE's lower back as EE attempted to twist and throw an airbag onto a rail mounted motor.
Mechanic was going to fuel drill at the bottom of east ramp when EE lost the brakes and rode truck down to bottom of pit where the truck flipped onto its left side.
During stemming process, EE was standing behind blast hole with EE's hand on top of shovel when skid steer operator proceeded to lower bucket to dump stemming. Employee's hand was caught in between bucket and shovel crushing EE's right hand.
Employee was walking around the Longwall drive area slipped and fell on right arm causing fracture.
Injured 6/20/17 pulling cable by hand & cable was entangled & jerked employee. EE did not receive treatment until 11/24/17. EE did not miss any work until 11/27/17 & became reportable 11/24/17.
Injured backed 1 ton truck up on a 1 ft high bladed berm on level ground to replace a U-joint. EE put the truck in park, set the brake, and chocked the front wheels. With the drive shaft removed, only the front brake and chocks were left to hold the truck on the berm incline and they slid 2.5 foot forward on the muddy ground causing shoulder bruising by the rear dual wheels.
While unloading sandbags from the back of a pickup, a laborer was injured when the driver of the pickup backed up without warning, striking the victim's abdominal area with the tailgate. This resulted in abdominal contusion, causing lost work days.
Employee was on a walkway on the crushing plant. EE ducked down to walk under a conveyor, and when ee stood up ee felt a ""pop"" in knee. It didn't bother ee until ee got home, when ee felt pain when trying to bend leg.
A 10 PASSENGER VAN (WITH SUPERVISOR INSIDE) WAS STRUCK BY AN OFF-ROAD TRUCK. SUPERVISOR ESCAPED THROUGH WINDSHIELD WITHOUT PHYSICAL INJURY BUT WAS BADLY SHAKEN FROM THE ACCIDENT.
Operating a powder truck employee had to negotiate a corner before descending a ramp EE switched to hand controls so EE could stand and see EE's load in the process of switching back to foot controls & sitting back down the vehicle came to an abrupt stop employee contacted the windshield with head & upper torso diagnosed with a concussion/restrictions issued.
The offside roof bolter operator on 3 section in #2 entry was struck in the back of the right leg by a piece of rock while roof bolting. The operator was transported to Raleigh General Hospital where x-rays showed a broken fibula, broken pinky toe, and cracked ankle. Operators return to work is unknown at this time. Operator worked for Lightning Contracting Services MSHA ID-8CV.
We are still unsure of what has happened. The employee is under Doctor's care until determination is made.
WHILE OPERATING CATERPILLAR LOADER, OPERATOR HIT A BUMP RESULTING IN LINGERING LOWER BACK PAID. INJURY TO LOWER LUMBAR. STRAINED MUSCLE TRAUMA.
Employee tripped while chaining cars together and caught self with right hand on the ground. This caused a fracture to hand and wrist.
Employee was descending dozer. EE stepped from dozer to stepladder. The ladder went sideways. Employee tried to catch self by grabbing hose on the dozer's lift cylinder, but EE's hand slipped, causing EE to hit the left side of head against the dozer filter and fell to the ground.
Worker was doing routine maintenance on skid steer bucket (welding). Chose to lift bucket and weld on it at an unsafe height while it was supported only by a chain instead of setting it on a block or welding table. Bucket fell on worker's toe while worker was welding.
Employee was getting on a 988 surface loader using three points of contact. While stepping up onto the bottom step, the employee felt a pop in the left shoulder. The employee sought medical treatment and was unable to return to work.
Leach operator bent down and felt right knee pop; employee was evaluated by doctor, and returned to work with no restrictions or time loss. A follow-up appointment with doctor including MRI showed torn ligament requiring surgery. No lost time, restrictions, or other treatment up to surgery, through surgery and first lost workday occurred September 28, 2017.
While walking down coal slope, employee slipped on metal grating causing employee to fall resulting in a sprain of employee's right knee. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was working on belt line. Employee was prying on a belt stand and strained left shoulder.
Employee stated was changing screens at the 8x24 location in the plant and felt something in the back as EE was maneuvering the screens into position. * Please note this injury was not reported until July 7, 2017.
Ee stepped off dozer, foot caught in grating and ee hyperextended left calf/Achilles when ee landed on left toe. Seen at ER, diagnosed with strained left calf. Follow-up confirmed diagnosis, ee released with restrictions site could not accommodate.
IE stated that while throwing over miner cable felt a sharp pain in lower left of back. Having sharp pain radiating down left leg to knee. IE was seen by Dr. on 6/20/17 and taken off work at that time.
Employee was walking along the 8 Hlg #1 belt and slipped and twisted right knee.
Rock had fallen from the working face into the conveyor of the continuous miner and was then loaded into the shuttle car. The rock then fell off the shuttle car, striking the employee's left leg, causing fractures to the femur, tibia, and fibula.
Liquid spilled off the lid of a 5 gallon bucket of oil while contractor was lifting it out of the containment area. The spilled liquid contacted EE's shirt and pants. As the day progressed EE said it was irritating on the skin.
The technician was installing a roll pin into repair link for chain and a small piece of metal broke off and was embedded into upper left arm. The wound caused bleeding which an EMT was able to get stopped before technician was transported by ambulance to the local hospital for care.
Employee stated was walking through a return mandoor at 6HD and when EE stepped through the door, EE felt pain in the right knee. This resulted in a strain and lost work days.
Explanation: The original date of injury was 6-16-2017, this employee continued to work until the date of 1-31-2018 at which time ee began missing work due to this injury. Description: While walking to the mantrip at the end of their shift, ee stepped into a hole and twisted knee.
Employee stated was operating the #323 Shuttle Car on the Unit 3 section. In the process of tramming shuttle car the employee ran over a rock in the roadway which caused employee to strike their head against the canopy of the shuttle car. This caused the employee to feel a pain in neck.
Setting a transmission in a 777 - boom kicked out, extended boom so it would work then transmission started to swing and while trying to keep boom from swinging with right hand felt pain in right shoulder - strain to shoulder.
A fully trained miner injured right shoulder when installing a rib bolt. A rock fell between straps striking right shoulder. The accident was not a result of any compliance issues nor were any protective guards involved.
Employee was on conveyor, 12'-14' high, lubrication rollers. Plant operator could not see the employee, blew the alarm (for all personnel to get clear of plant) and started the plant. Employee said employee did not hear alarm, fell, fracturing employee's left wrist and sustaining a displaced fracture to the right arm.
While sounding and scaling the roof, employee drew hand back quickly to avoid a rock that was dislodged straining right shoulder.
Employee was walking to talk to motormen when employee stepped on the corner of a track tie and felt pain in employee's left foot. Diagnosed with a broken 5th metatarsal.
Bit on end of steel was stuck and employee was hitting steel on pot of roof bolter trying to knock it loose. Employee struck their right pointer finger on the pusher plate causing a laceration which required stitches to close.
Employee was using a wash down hose at cross cut 37 on West Main belt when employee tripped on hose and fell on left shoulder, neck and back area.
While carrying a vulcanizing press with another employee the individual felt a sharp pain and tightness in lower back. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.
EE was resetting miner breaker turned to walk away & stepped on a nail approximately 3"" long, penetrating about 2"" into right foot. EE went to ER was on crutches, foot swollen, red, & tingling. EE followed up with doctor & returned to work full duty with no restrictions on the next scheduled workday (06/21/2017). EE missed two scheduled workdays.
EE was disposing wet dynamite caps. The caps were put into a designated area, lit on fire, and EE left the barricaded area. EE returned to the barricaded area, leaned down to relight the fire and at that point the caps detonated. EE was wearing safety goggles but EE still received minor burns to the face and a small fragment of steel in right shoulder.
Employee was knocking stopping with scoop bucket then went to get out of the scoop and an 8"" hollow block fell hitting employee on left hand. This resulted in a hairline fracture and 8 stitches to cut on the palm of hand.
EE was bolting on the longwall face when the rib rolled out and pushed the EE into the bolter causing a laceration and puncture wound to EE's right elbow area. The EE had surgery to clean out the puncture wound.
Road was wet. Driver said that EE looked down and when EE looked back up EE was at the edge of the road and ran up on the side of the safety berm, which knocked the wheel back into fuel tank. Driver sustained cut on head from hitting light and bruised ribs.
Employee was descending the access ladder of a front-end yard loader during a pre-shift inspection when ee fell, fracturing left ankle.
Employee was in the process of replacing polyurethane screen when they hit it with a hammer. Hammer bounced off screen and the handle caught employee's right index finger.
The employee was struck on the left arm by a piece of steel while hammering on a loader bucket tooth with a steel hammer during maintenance.
Employee was picking up a piece of metal next to screener. Received a puncture wound without foreign body to the right hand.
Employee was carrying rock dust to dust belt line after a belt move, and as they were walking beside the 6"" waterline that was being pulled into place to be installed, a fire valve caught the lower-back part of their left leg causing them to fall.

Employee was pulling a miner cable pull rope to attach it to the tail boom of the miner and fell after becoming entangled in the miner water line. Employee fell backwards on to left buttocks. Employee is complaining of pain near tail bone radiating down left leg. Employee continued to work until 6/21/17.
Helper parked truck by drill on other side of shot berm. Then crawled over berm to get to the drill. As EE was going over the berm, fell and broke arm. Helper should have walked around to opening on berm instead of going over it.
Employee was rolling scrap tires from one side of a cross-dock to the other. Employee reports feeling a sharp pain in left arm, shoulder, and ribs. Dr. reports contusion of left wall of thorax.
While performing repairs on a conveyor belt, employee injured left knee.
Employee was using portable drill to drill holes in the mine roof to install trolley wire hangers. The drill dropped suddenly, striking the employee's right elbow. They finished the shift and filled out an incident report when they left the mine. Went to a doctor and was placed on restricted duty. Worked 2 days and then saw another doctor who placed them off work.
While operating the continuous miner, a loose rock fell from the roof and struck the employee.
Employee was swinging a 3lb hammer overhead to loosen the stone box feeding the syntron on the tunnel belt at plant #1 and strained a muscle in EE's right side.
Employee was side bolting on the miner in 2 East Mains. They were beginning the 4-5 crosscut at 86 wall. After they installed their first strap, the side bolter traveled outby just as the top inby corner fell out. The rock landed on their left foot. Diagnosed with a dislocated foot.
The employee stepped off a small pile of gob and fell onto right knee. The accident occurred on 06/12/17. The employee continued to work until 06/21/17 when ee went off on restricted duty. This accident is under further investigation.
Employee was working to clear jam in primary crusher. Employee was standing with feet at different levels and lower foot slipped causing the left knee to strike a metal bar. Employee finished the shift but knee later swelled up and ee was unable to work on 6-13-17. Employee's PCP set up appointment with Orthopedic doctor on 6-20-17.
OP4 bent over to lift a six inch hose so it could be relocated for the portable sump pump. When EE attempted to resume an upright position, EE felt a sharp stinging pain in the middle of EE's lower back.
Injured was carrying a bag of rock Dust into a power center location thru a door and felt stabbing pain in back. Emergency Room Dr. determined strained muscle in back and could return to regular job of Fire Boss on next shift. Injured returned to reg. job next shift and worked thru 6/20/17. Employee began missing work on 6/21/17 due to this injury.
Employee was operating a 40 ton articulating truck moving overburden before the dragline begins. As employee entered the dump area, the front wheel went into soft ground or a rut, causing the employee's upper body to sway in the seat. The employee felt a pop in employee's neck.
Employee was working at a surface PC room. While working on the PC they decided to open a garage door, as they attempted to open the door they bent over to grab the middle of the door, as they bent over, left foot caught on some grating causing them to fall forward hurting shoulder. Recent medical evaluation has placed employee on Lost Time from the work place.
Employee picked up miner cable & water line to move it out of the way. When they turned & pitched the cable they strained their lower back causing muscle spasm. Employee went to local ER and has strained the muscle in their back causing spasms. Employee is off work and has lost time.
EE and another ee were standing on a wooden cover on a belt built to keep contamination off of the conveyor. EE was starting the plant. EE communicated with them ee was starting the plant. EE climbed of the belt and another ee walked 30 feet down the belt not expecting the belt to be turned on. The belt started and ee fell off about 4 feet
Dozer operator was ripping on the quarry floor with a CAT D-10 when they hit a large rock, due to the nature of the ripped material the dozer was on uneven ground and the employee jarred their neck and mid back due to the side to side motion. The employee was diagnosed with a neck strain and thoracic back pain, they were prescribed medication and given modified work duty.
The employee rolled ankle while walking.

A fully trained miner was using a Z-Drill when EE felt a pain in neck and back. The injury was not the result of any compliance issues nor were any protective guards involved.

Employee had finished assiting removing a length of steel from the rack. Employee was standing still on the drill walk way and as employee went to take a step employee felt employee's knee pop. First responder assessed and later assesed knee cap possibly dislocated
Employee stated was stepping off an I beam on the #1 belt drive landing on left leg causing trailing right knee to twist. Employee finished shift and did not seek medical treatment at that time. EE sought medical treatment on 6/16/17 and began missing work on 6/18/17.
EE's right hand thumb got smashed between a 50 lb rock that had slid down as EE was stacking rock.
Employee was using a pry bar to help open a barn door dump bed and when door opened employee leaned forward straining lower back muscles. Stiff/sore lower back & right hip.
While preparing the pad to load hauling trucks; the side wall gave out causing the excavator to slide down approximately 12 to 15 feet; once the excavator got to the bottom, it tipped on it's side.
Lifting suction pipe. First thought had pulled a muscle in lower groin, but 2 weeks later it popped out.
Employee stated employee and another employee were removing a bearing from the top of the load out. As employee pulled on the bearing employee felt pain in employee's middle back. This resulted in a back strain and lost work days. The employee began missing work on 7/11/17.
Employee was resetting a damaged post. Employee's partner noticed dribbling from the ceiling and yelled at employee to get out. Employee got up and started moving towards the opening. Slab fell from the ceiling and caught the employee underneath.
Employee was walking down the slope belt with a 48""x32"" piece of guarding for a cross under when employee felt a pop in employee's right knee.

EE was walking through crosscut to make the on shift of the faces. EE stepped on a crib tie, the crib tie rolled and EE's knee twisted and buckled. EE had surgery on knee on September 21, 2017 and was taken off work at that time for recovery.
Changing tire on equipment. The tire fell striking employees foot.

Employee was assisting another employee to remove a mandoor from a stopping. They knocked the blocks around the door out and the employee was going to attempt to pull the door out. In the process of the employee pulling the door out a cinder block fell from the above where the door was positioned and struck the employee on the hand.
While hooking up a rigging chain to a steel plate that was being prepared to be moved, the employee yanked on the chain to remove the slack, which caused the plate to fall from a leaning vertical position and hit the outer surface of employees left leg knocking ee to the ground.
Employee was descending a ladder with a piece of material inside raw mill. One of the ladder legs shifted causing employee to lose footing and fall down. Employee was taken to hospital and diagnosed with rib fracture and fracture of transverse process vertebra.
The employee was walking to employee's locker to put employee's boots on and slipped in water that was tracked from the shower.
IE was driving a mantrip on the 4th SE travelway when they fell asleep . A passenger yelled and they looked up as the mantrip hit the rib, broke the 4"" discharge waterline on the opposite side of travelway. The discharge waterline came through the windshield striking IE causing a laceration of the left triceps.
Employee stated while loading structure, ee strained left groin area. Employee finished shift and did not seek medical treatment at this time. After arriving at home, employee called wishing to seek treatment. Employee visited ER and received X-rays which were negative and discharged. EE visited PCP on 6/7/17 and was given time off to assess possible pulled muscle.
The injured employee was attempting to hang the miner cable on a ""J"" hook. The ""J"" was just out of reach and they missed the hook. The cable slipped from their hands and struck them on the left shoulder. The initial ""File Only"" report was completed on 06/07/2017. The injured employee sought treatment at a later date. The incident became a Lost Time Accident on 07/10/2017.
Employee was dragging wire mesh to #2 entry, when foot hung on miner cable causing employee to fall on the ground, over extending left knee. Employee started missing work on 08/24/17.
On 6/5/17, employee stated arms were sore from pulling pins from a haul truck bed. During conversation said injured arms on 5/28/17 loading cable. EE was taken to the doctor and taken off work. On 6/8, withdrew the claim. In July reclaimed the injury. WC TPA notified site 8/8/17 they were accepting claim.
EMPLOYEE WAS PULLING THE FUEL HOSE OFF OF THE DREDGE WHEN THEY FELT A POPPING/BURNING SENSATION IN THEIR RIGHT SHOULDER.

While lifting miner cable employee experienced a burning pain in abdominal area. Employee continued to work until doctor took ee off work on 7/26/2017. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency, attitude, or machinery.
Employee cut left hand while attempting to remove a plastic wire tie.
Employee went to pick up tool bag and felt severe pain in lower back area.
Miner was stepping off of the last step on the end dump and fell to the ground landing on back.
While bolting, EE twisted at the waist and felt something pop in EE's lower back. EE then had pain down EE's leg and was unable to straighten EE's back. EE continued to work EE's regular job duty without restriction until 01/29/2018.
Day shift production crew was heading to the section. An electrician pulled a motor into the run-around on main line 2. The employee failed to throw the switch and the on coming mantrip took the curve into the motor causing the injured employee to strain neck and back.
On 6/3/2017 an incident occurred when a track switch was not thrown back to the straight. The On coming day shift crew did not notice the switch thrown to the curve and the mantrip took the curve striking a parked motor. The employee suffered a back strain as a result. Note: Employee did not go on light duty until 6-13-2017
EE was helping to change take up pulley. Hoist broke and pulley/EE fell. Pulley hit the EE in the right hand/thigh, and across the chest. EE was taken to the doctor and released, no restrictions. On 6/8/17, EE came to work and complained of chest hurting, went back to doctor, and doctor wrote EE out of work until 6/12/17. EE missed 6/9/17, thus making accident reportable.
Installing roller on a D-10R dozer, using sledge hammer to knock roller loose, a small sliver of metal clipped off roller, penetrating employee's upper right thigh causing laceration. Note: employee returned back the next scheduled day of work 6-6-2017.
While running cable employee was lying on is back, a piece of draw rock fell out 8""thick 2'x2', striking right upper leg. EE received a small cut & bruising. No stitches. EE has loss time.
EE was helping unload a gang box out a pickup to the shop floor and carrying it backwards to set it down when EE caught left knee on a ratchet strap that was holding the belt press together. There was four other people helping to unload the gang box.
Miner in 3400-048 Raise, tripped and hit knee against footwall, small cut. Infection, miner off work for treatment of infection.
Ee saw doctor on 06/27/17 and reported an injury. Ee stated to doctor that ee was operating a loader in mid-May and hit a pothole and the cab slammed down; ee struck head on the roof of the loader. EE was diagnosed with post-concussive syndrome and put off work. Site immediately began investigation which is still ongoing.
Employee stated was loading timbers into bucket of scoop and felt pain in lower back. This resulted in a sprain and lost work days.
While individual was getting down from service truck, they slipped and hit right knee against steps. Follow up doctor visit/diagnosis on 6/15/17 indicated individual could not return to work under doctor restrictions.
While lifting a screen strained employee's back
Employees shift ended at 7:00 p.m. and at 7:10 p.m. while employee was leaving the facility on personal motorcycle, employee made contact with an ecology block causing an accident. Employee received minor abrasions and a torn calf muscle in employee's left lower leg.
A fully trained miner bent down to pick up a roll of air hose and when EE standing back upright felt a pain in back. The injury was not a result of any compliance issues nor were any protective guards involved.
Employee was training another employee on operations of the battery coal-hauler, as first employee was talking to the second employee first employee slipped coming in contact with the coal-hauler causing fracture to leg.
June 1st EE was hooking up Ferric cars outside of chlorinator building when they felt pain in back; Did not report incident. June 6th EE went to Emergency room & still had not reported it. On June 7th EE called spvr reporting that they had a personal issue. On June 8th EE reported to clinic that they had an incident on June 1st & entire back was in pain from incident.
While performing routine maintenance on water pump, employee smashed finger while trying to remove guard resulting in a laceration on the tip of the right thumb.
On 6/1/2017 Employee stated while washing the bottom floor of the plant - they drug the hose out the door, misjudged the single concrete step and fell on their left shoulder. On June 2, 2017 employee was examined and diagnosed with a torn muscle in their chest. Employee continued to work full duty until 8/9/2017 when surgery was performed to repair.
Employee was changing tire on right rear of man trip. When employee attempted to pick the tire up to put on man trip employee said employee felt pain in employee's lower back. Employee continued to work until 6/20/17.
Employee was replacing bits on the headgate shear drum located in the No. 4 entry when a rock fell out of the top, from between bolts, hitting employee on back and legs causing fracture to left lower leg.
While cleaning out area to change a dog bone on the longwall face and waiting for supplies, a piece of the face rolled out pinning the employees foot against the pan. This did not involve compliance issues. Personal protective equipment was being used and this did not involve job proficiency, machinery, or attitude.
An employee was descending down the 740B articulating truck ladder when ee slipped and fell. The employee fell to the ground and in the process injured self. Urgent care facility diagnosed employee with a sprain to the back and left knee. The employee was required to see a specialist before returning to work.
While operating roof bolter employee was struck by a glancing blow from a piece of drawrock. Employee finished regular shift, continued to work and did not go to the doctor until 6-14-17. Employee was released full duty. During a follow-up visit on 7-24-17 employee was taken off work pending an MRI. It is at this time the incident became a reportable injury.
Employee was going through doors on track when EE hit head on beam at overcast in between doors at 1C belt.
Employee was struck by a piece of roof/rib that they were drilling through causing them to fall forward and strike their face on the side of the miner. This caused a laceration to their right eye. 6 stitches
On July 16, 2020 employee reported that EE experienced a shoulder injury in June of 2017 and EE wanted to seek medical attention (employee did not report injury in 2017). On August 24, 2020 MRI results revealed a labrum tear consistent with an old injury. EE is scheduled to have surgery on September 11, 2020. EE is currently working full duty until surgery date.
The injured employee was pulling on a section of 250 MCM power cable and connector on an Addcar being installed on the Launch Vehicle when EE felt a ""pop"" in the right shoulder. This incident was not reported until 7-4-17.
Employee was in the process of repairing the 027-07-2 belt. Employee was transporting the cooker platen from the belt to the cart. While employee was lowering the cooker platen to the cart, felt a pop in right shoulder. The employee was given restrictions on 7-10-17.
Employee lacerated finger while removing a stone wedged in palletizer. Received stitches in right thumb. Will return to work for light duty after 4th Holiday.
The employee was entrapped and sustained injuries while acting as a first responder to 2 fellow workers who had become entrapped at the primary surge pile.
Employee was feeding the jaw crusher with rock from the stockpile. When employee was backing away from the stockpile with a bucket load of rock a large rock rolled down the stockpile and hit the left corner of the 966k Caterpillar front end loader. This caused employee to be jarred and lower back was injured.
EE was setting pipe in a trench when EE lost footing due to the muddy conditions. EE stuck left hand out to prevent self from falling causing EE's shoulder to pop.
Employee was lifting a stone and felt discomfort in lower back.
Setting center pin in place, EE's fingers were underneath the head pin. When pin slid into the bore the EE's fingers got smashed between the head of the pin & bore.
EE lifting drill steel onto tractor and felt pain in groin area. EE did not seek medical treatment at the time of incident. Medical treatment on 7/11/17 placed employee on restricted duty pending evaluation and treatment for hernia.
Miner was helping repair pump when fingers got pinched between the pulley and the belt.
Pulling cables to re-position dredge. Dry, sunny day.
EE and co-worker were opening valve. Valve was stiff. EE reported pain in left shoulder after. EE was taken to the ER was released back to work. EE was on last schedule work day. EE reported to work on 6/28/17 reported that ee still had pain in shoulder. EE went to PCP on 6/30/17. EE report on 7/3/17 to us that ee had been taken off work and referred to another physician.
A miner was loading ground support bolts, EE leaned down to load supplies and felt a sharp pain in lower back. Self-treated until leaving site for R&R on 06/29; 06/30 & 07/01/17 saw a chiropractor then physician on 07/03/17, whom placed EE on physical therapy and Unable to return to work from 07/03/17 - 07/11/17. EE's normal schedule is 2:1; with return for work on 07/06/17.
While placing a shank on the 12 shovel, employee lifted and wiggled the shank with left arm while shank was attached to the crane. During the process of final positioning, employee stated they felt a pop and sharp pain in left elbow. Employee had surgery on 8-16-17 making this a reportable case.
Employee attempted to free a lodged rotor on the Terex crusher, entered the feeder, dislodged the rock, the rotor then turned trapping EE in the machine.
Trimming a piece of rubber to fit a conveyor transition chute with a utility knife. The knife slipped and cut the top of the left hand.
The employee was physically pushing ½ of a 16 foot airlock door to open it when severe pain occurred in their lower back area. The metal airlock doors are located at 12 crosscuts, inby the no. 5 return air portal and separate the main intake and return air courses. The doors are 16 ft. wide. A lot of air pressure is present at this location.
Employee was using a grinder in the shop with safety goggles on and a piece of foreign matter flew into right eye. The supervisor told employee to flush their eye out and seek medical treatment and they refused both saying they would get their spouse to do it at home.
Employee was fabricating a girder, when one of the I-beams tipped over onto said employee's ankle. Result of the accident employee's ankle was fractured and corrected by surgery that day.
While exiting a tow tractor, slipped in an area of wet conditions. EE grabbed the tractor to keep from falling and felt a sharp pain in EE's left arm and shoulder. EE completed EE's shift before reporting the incident and continued working. Initially reported as first aid only, began missing work due to cervical fusion surgery on 08/15/2018.
Employees were changing out dipper teeth. Employee placed hand on boom to aid in getting up, when operator began to retract boom, catching finger in knuckle. Employee diagnosed with fracture and received sutures. Currently off work.
Employee stated employee was installing roof bolts in the #4 Entry and as employee walked toward the back of the roof bolter a rock fell from between bolts and struck employee on the back of the head/neck/shoulders. The rock measured approximately 4'L x 2'W x 0"" to 2""T. This resulted in a strain and restricted work days.
Employee stated they and another employee were carrying a 20' joint of water line into the shop when the employee felt pain in the left shoulder. This resulted in a shoulder strain and lost work days.
Finger caught between the wrench and the mast of the drill ee was operating
IE was standing checking rail car doors to make sure they were latched. IE alleges that right knee twisted/hyperextended as IE was standing there. IE did not report this incident until 06/26/2017. IE continued to work up until 08/10/2017 when IE had right knee surgery. Surgery was arthroscopic partial medical meniscectomy and debridement of plica.
On 6/13/17 the EE stated that they stepped in mud while walking. When they pulled their foot from the mud, the knee began to hurt. The EE came in on 6/19/17 and stated that the knee was not getting any better and wanted to go see the Dr. The EE was taken off work on 6/19/17 pending a MRI. Results of the MRI revealed a rupture of a Baker's Cyst.
Employee was in the splitting bay using a trimming hammer and a small piece of stone flew off and wedged under employee's skin.
An employee was tapping bolts into the 12"" flange for the waste pump with the hammer in EE's right hand when the hammer slipped and EE hit left hand, fracturing EE's left middle finger.
While adjusting a hydraulic cylinder with a portable pump a quick coupling fitting malfunctioned leading the crew to believe the pressure was bleed off when it was not. while removing the quick coupling from the cylinder it flew off and hit the miner in the arm causing tore muscle and damaged artery. The miner is expected to miss 4 to 6 weeks of work.
Two EEs were carrying a piece of screen media up to the feed end of the scalp screen. One EE was walking backwards, injured employee walking forward. IE stumbled & stepped back a step, in doing so rt foot & leg went into an access hole for a chute gate lever. IE's lt leg went to the floor of platform, rt leg went thru the hole. IE's groin area struck toe board around access hole.
EE was preparing to rig a rotary hammer for replacement in the mill when EE inadvertently knocked another hammer off of the edge of the pallet and it rolled onto EE's right foot. EE was diagnosed with a small toe fracture on right foot. EE was wearing steel toe boots at the time of the incident but the injury was outside of the protection of the toe cap.
Team member lost grip on a pneumatic torque wrench causing it to slip in hand. When it did it caused the team member's right thumb to hit the side of the wheel that the member was working on.
Miner states their knee buckled at ~5:30am while dismounting CAT 775 haul truck at end of night shift on 06/11/2017. Miner did not report the incident and went home. Miner called their supervisor at ~4:30pm on 06/11/2017 to report the right knee was swollen. Miner went to local ER diagnosed with knee strain. Miner was released back to work with restriction 06/12/2017.
Employee was late reporting a job-related injury (6/11/17) to plant nurse. On the morning of 6/18/2017 employee reported infected callus to nurse. Upon observation, caustic burn to outside of right big toe noted.
Injured employee entered an area at screen 2 unannounced while work to remove deck was being performed by others. The corner of a screen deck that was being removed contacted employee's left foot when it was cut free.
Employee was working Saturday and Sunday driving on rough terrain when employee's back started hurting. Employee was off on Monday and when employee came in on Tuesday, employee hit a bump in employee's haul truck and hurt employee's back again.
Employee was pulling pallets with an air winch when EE's finger got pinched between the wire rope and the winch spool.
Employee fell from a ladder on the plant at the remco 3000 crusher at the back-end.
While loading material out on the 7000 bench of the RE3 layback, the operator, of LDR003 encountered a hard spot in the floor causing the loader to articulate hard and ""jar"" employee's back.
An employee stepped off of a forklift and twisted their left ankle. Due to continuing pain, the employee was seen on 7/5/17 and was given work restrictions.
Three mechanics were repairing a leak to a spool piece on a sulfuric acid line. LOTOTO was done; Sulfuric acid ponded next to the line; Ee's were removing the spool when a piece fell into the ponded acid. EE jumped to move out of the way and fell into ponded sulfuric acid; EE sustained burns.
Member bent over to pick up a blue flag (Used to mark the R x R tracks of cars that are not to be moved - Weight of it is ~ 10 lbs.) Member stood up and felt a sharp pain in lower back
A Coarse Crusher MTM was attaching a 4 legged chain to the concave jig using a 4 step Cotterman portable stair case. As EE was descending the stair case, EE felt the stair case tip back, which startled EE. EE then pushed self back off of the stair case, landing on right foot.
Employee was opening a maintenance trailer door. The hinges on the door failed causing the door to fall. Employee attempted to catch the falling door and suffered a strain in employee's left groin area. The employee waited until 6/12/17 to seek medical attention at which time employee was put on a 25 lb lifting restriction.
While changing an elevator bearing, the miner went to assist the other miner on the opposite side of the elevator. EE's body was then out of position and they noticed a shoulder problem when upper range of motion was attempted. Miner had surgery on 06/25/2018 since shoulder did not heal properly over time.
Employee stated that during a power outage ee was trying to close a gate that normally opens and closes with an electric gate opener. Gate was switched to manual setting, in process of closing the gate the employee strained left shoulder.
Employee was attempting to remove excess build up in duct work of the pre-heat tower. The employee noticed loose material on the walls and ceiling. When the employee exited the duct, the material sluffed off of the wall and ceiling contacting employee's leg. Employee taken to ER, no fractures but bruising on leg and foot.
Employee placed employee's hand on the rock in the splitter to keep it stable when Criss cut splitter split the rock the piece fell on employees hand.
As worker was climbing in to the flatbed of a truck ee felt a pop in left foot. EE continued to work and declined treatment at the time of injury. EE did not think it was too bad and thought it would get better. Pain gradually worsened over several days. On 6/11/17 ee was hurting bad and decided ee needed treatment.
Employee was lifting a truck tire/wheel to reinstall on pickup truck and felt a pop in EE's back.
EE was attempting to attach a chain from a front end loader bucket to a trailer to lift it up and remove jack stands. The front end loader moved forward and ee stepped back, stumbled and fell on back. EE has a cracked T7 vertebrae.
Employee was installing a 7-8 foot long water pipe and was trying to start threads into a 90 degree elbow. The opposite end was resting on a metal box. As ee turned the pipe trying to start the threads, the pipe rolled off the top of the box causing the pipe to shift and mash the employee's finger between the pipe and corner of the equipment.
Employee was dismounting a forklift, slipped and strained right leg.
EE was trying to move a stone into a loader bucket. All was ground level. No lifting involved. When ee gave a last push - ee felt a pop in lower back. EE sprained back.
Employee was relocating the rope lines to the dredge and pulled a muscle.
Employee was exiting the cab of the water truck when employee felt employee's left knee pop.
After ascending and descending a ladder that was to a platform approx. 6 1/2 - 7 feet in height the injured employee felt pain in employee's left shoulder.
Employee was trimming a newly installed grease line for a bearing installed on a tail pulley. When employee cut through the grease line ee cut left index finger. EE was seen at the occupational on 06/30/2017 where ee received butterfly bandages. EE was released to restricted duty on 06/30/2017 with a follow up appt. on 07/06/2017.
Employee just finished servicing a piece of equipment and was walking down a small set of steps (3 steps) when their foot slipped, and they fell onto concrete floor on their backside. Employee extended both arms/hands beneath them to break the fall, spraining both wrists.
Miner alleges being struck by a rock in the upper left back area
On 6/29/2017 employee was operating a loader and strained back when the machine drove through a rut. Injury did not result in reportable restrictions until 7/10/2017 due to employee on vacation for the holiday. Spoke to the MSHA office of Injury Illness and employment.
Operator checking on a pump not discharging. Shut power off and removed the fill plug to wash out the mud. Hot slurry blew out and hit wrist, chest, neck and chin. Resulting in 2nd and 1st degree burns. Released to modified duty and given a prescription.
Employee was walking backwards talking to another employee in the equipment parking area. As EE turned around, EE turned into a fill pipe on the side of the parked water truck. The pipe struck EE in the right upper rib cage shoulder area causing EE to fall. Employee complained of right shoulder pain after incident.
Employee stated was prying on a rock while hand scaling and felt a pain in back.
Cleaning off paver stone and got stone dust/debris in eye (said was wearing safety glasses that was provided).
While using a chain come-along, to lower a rubber tire belly pan, the hooks became unhooked, causing the belly pan and come-along to free fall to the floor (1.5 ft). The employee's finger was pinched between the come-along and the concrete floor.
Employees were lifting a feed tube for the impact crusher when one employee lost employee's grip causing the tube to fall onto other employee's hands crushing the ends of both middle fingers on each hand.
Laborer was walking on a concrete walkway and stepped on a rock causing employee to twist employee's knee. The result was a strained knee which causes minor discomfort when employee walks on it.
Operator was instructed to clean out G-15 dust collector with the vacuum truck. EE was returning from evaluating the job and ee began descending down a steep set of stairs. The operator's left foot came down on a step 3-4 inches below what ee expected. The jarring impact fractured the employees heel.
EE was pulling on a wrench trying to remove a plug from an L250 Volvo Loader. While pulling EE's left shoulder popped. EE was taken to clinic by another EE. The plug was real tight causing EE to pull extremely hard.
While loading the empty core tube into the rod string the employee caught employee's thumb and middle finger of employee's right hand between the spindle bushing and the drill pipe.
Initially reported FA. Became RD 7/7/17. Employee leaned against handrail that moved. Employee struck hand against the electrical cabinet keeping balance.
Employee stated that while taking down the CM Cable from a roof bolt plate, the hook sprang off of the lip of the plate and swung around striking employee in the nose. The hook ended up lacerating employee's nose, which required 11-12 stitches. Height in the area was 52-53 inches, cable was under some tension, employee was dropping cable by self.
Driving haul truck with arm resting on arm rest. Truck bounced, jarred shoulder.
Tripped while stepping over elevated power line. Strained tended in right arm around elbow.
Employee was using a packer pole while loading a bulk truck. Employee strained a mid back muscle while using the packer.
While using a steel shaft to drive pins from a rock breaker, employee was holding the shaft down on to a jack stand that supported the shaft. Another associate was using a sledge hammer to drive out the pins. The associate missed the shaft & struck employee's left hand which fractured a bone.
Injured was bringing up the pot to connect to the split steel when the drill steel caught the chuck and buckled, and struck the injured on left thumb. Injured was taken to clinic for x-rays where it was discovered injured had dislocated thumb.
Miner was moving stones, by hand, from a loader bucket to place on a conveyor. A stone shifted and left index finger was smashed between two stones resulting in a broken finger bone.
Employee was moving a piece of angle iron in a walkway 6""x2""x4', while picking it up and setting it down the material got away from them and they got their left middle linger smashed when it was caught between the metal and the surface. Split finger nail, no other injuries except sore.
Employee was cleaning out clinker cooler by tossing large chunks of material onto a chute take the material outside. A gust of wind blew back through the door bring dust back inside into the face of the laborer. Laborer has dust mask on, safety glasses on and indicated had goggles on.
While walking, ee slipped on clinker landing on left forearm & wrist.
The employee was running to turn on computer screen for the slurry and rolled their ankle.
While backing haul truck under shovel bucket, the bucket swung forward and struck headache rack of haul truck resulting in impact, jamming operator.
Employee was changing out Idlers on Conveyor systems and noticed pain in the lower groin area.
A bulk loader fell from a tanker truck. Their harness arrested the fall and the swing fall carried them into the structural steel in the loading alley. No injury from the fall; Contusions to their legs from the impact of the swing fall.
Miner was climbing on feeder, changing liners. When in an awkward position miner felt a sharp pain in lower abdomen/groin. Miner was diagnosed with ""STRAIN OF MUSCLE, FASCIA AND TENDON OF ABDOMEN.""
Employee slipped and fell in the Kennecott dry this morning. There was a puddle of water on the floor near the sink where the employee slipped on.
Employee was attempting to pull a gate closed. While attempting to force the gate closed the employee felt a muscle tightening in EE's back.
While attempting to lift a 200 lb roll of belting with another employee injured employee strained back
Employee was working on changing a bearing in the plant. Their foot slipped and they rolled their ankle resulting in a sprain. Employee was placed on light duty.
Employee was operating an industrial vacuum to clear out dirt from around a pipe that needed repaired. As employee was using the machine, EE got the end of the suction line too close to the ground causing it to abruptly suck down to the ground. This abrupt movement caused the employee to be jerked down too. Employee strained lower back as a result.
Employee was injured while the haul truck was being loaded. Their left shoulder/lower neck injured when they were jostled around in the truck cab while rocks were being loaded in the bed. Type of injury is yet to be determined, but appears to be a type of strain.
Working on conveyor frame when employee's finger was pulled in between the two frame flanges by their gloves.
Electrician was pulling tool bag from the bed of the bucket truck. As employee turned to walk away, the bag slid off the bed and jerked employee's left wrist down, chipping a small bone fragment from a wrist bone.
Employee stated that EE bent a bolt against the t-bar of the roof bolter to get it up into the place so the bolt could be installed. When the employee got the bolt started up into the hole EE was going to attempt to bend it back to straighten it up. In the process of EE bending the bolt back EE felt a pain and EE's shoulder popped.
Employee was in a haul truck and hit a rut in the road, felt a strain in neck. Employee was referred to the clinic and placed on modified duty.
Employee was shoveling material on to the hot skip. As employee was lifting shovel, they felt a pop in their back.
Employee on bed of pick up truck picking up empty paint cans. When employee attempted to exit bed of truck, EE elected to step off on the tire side, as opposed to back. The back had a step provided by bumper. As EE exited the side, right foot on tire, left foot caught the wooden bed rail, as EE unhooked the foot, EE pulled it back, landed on left foot awkwardly, twisting knee.
The employee was going down stairs, when they over stepped, and felt their right knee lock up.
WORKING ON CABLES FOR A TRANSFORMER, UNDER A CRAWL SPACE. EE EXPERIENCED BACK PAIN AND MUSCLE SPASM.
While cutting rubber roll ""nose rubber"" with utility knife, the knife slipped and cut employee's left hand (palm)
While attempting to dislodge rock from feeder, the rock shifted and pushed the pry bar into the handrail pinching left pinky finger between bar and rail.
While attempting to remove a machine guard to tighten drive belts employee experienced ""pop"" and tingling sensation in left shoulder.
Employee was standing on a ladder removing a deck from a haul truck when the exhaust pipe shifted and the employee jumped approximately 5' to the ground striking right foot.
Two employees including one supervisor was in the Quarry tunnel attempting to raise the gate for the tail pulley. After repeated attempts, the wall went up and jammed one employee's right hand between the pulley and the wall of the tunnel. It took about 3 minutes to pry their hand out from being squeezed.
Employee was moving a portable gas heater in the cooler basement and strained their back. Employee was pulling the heater backwards when their foot slipped in some pellets and employee felt a sharp pain in their back. The heater has wheels on one end and hand rail on the other end to be able to maneuver them around.
EE was using heavy pry bar to clean material from clay shredder, when strain occurred. EE did not feel immediately necessary to be evaluated medically. approximately 1 week later, physician diagnosed as thrombosed external hemorrhoid. Procedure performed to drain, employee prescribed restricted duty.
LIFTING TANK CAR LID TO CLOSE AND SEAL MANWAY WHEN EE FELT A SHARP PAIN IN LOWER BACK. - LUMBAR SPRAIN.
EE was rolling up curtain when a piece of draw rock fell striking EE on the Neck and shoulder area.
EE was lifting a hydraulic rock splitter from a rock when EE felt EE's right shoulder give out. EE was asked if EE wanted to do another job, EE said ""no"" EE was alright. Monday 6/19/17 EE said EE's back felt like it was going out, at that time went to a less strenuous job. EE's on restricted duty not lifting more than 10lb, PT for 4 weeks and then re-evaluation.
Subject stepped out of work trailer onto a loose rock causing the right ankle to roll. Ankle went right, body continued forward resulting in a sprain of the ankle.
Employee was removing an elevator chain pin that was wedged between the idler and elevator casing. While pulling the pin, the employee placed right hand over the end of the pin. Another mechanic who was assisting in this job was jogging the elevator. The elevator rolled back wedging the pin against the floor and the employee's right hand.
EE was removing a guard on the counter weight of C4X when the guard pinched right hand middle finger.
While roof bolting drilled the first four feet and grabbed a four foot extension. While they were lowering the drill pot they had the extension in their right hand when it caught beneath the pot and on top of their right foot. The steel poked through their boot and into their fourth and fifth toes. X-rays revealed a fracture to each of the toes.
Employee stated that employee was carrying a roll of fly pad and slipped and fell back landing on the roll of fly pad straining employee's lower back.
Employee was operating a bagging machine when EE opened a machine guard without locking out the equipment and the employee was struck by automated arm that places bags on equipment to be filled with materials.
Employee alleged that as they began to climb entry ladder of a haul truck they placed their right foot on the first step. As they took the weight off the left leg the left knee popped and caused pain. EE was seen at a clinic where restrictions were placed on their work activity. No direct cause was identified.
On 6/17/17 Employee was swapping compressed gas cylinders when employee lost balance and was struck by cylinder on the lower leg. Employee wound was treated contusion and abrasion with first aid. On 7/6/17, employee was still experiencing pain and wanted it evaluated. The employee's wound had gotten infected, employee given Rx for antibiotics
Employee called stating fell at work and twisted right knee
Miner was walking through pit and tripped. Landed on knee. Went to doctor as precaution.
Employee was helping carry a wet, muddy timber and they felt a sharp pain in their lower back. They worked restricted duty until they were fully released on 06/22/2017.
Alleged slipped in shower, injuring back. Did not report til end of shift on the next day
The employee was beating a drill steel to remove dust when employee aligned to receive a Ganglion Cyst. We feel that this type of work activity did not create this injury so we are submitting this as a occupational injury. The injury was on 6-13-17 and the employee continued to work until 8-2-17
Injured states ee was installing a starter on work truck. EE was underneath truck when ee was pushing self up by elbow to reach the top bolt of the starter. This caused left elbow to develop a fluid sack.
Miner was in the shop using a hand-held grinder to prep a metal surface. EE moved to another section and felt pain in lower back. EE was taken to a medical clinic, examined, given anti-inflammatory meds and released to light duty work. On June 19th EE returned to the clinic and was released to return to full duty work.
Employee ran to activate stop button on conveyor as EE saw it choking up. Noticed back hurting afterwards.
Employee was outby on the E28 Longwall belt checking dollies. EE crossed the belt to move a chain off the dollies. When EE returned to the walk side of the belt, EE tripped coming off the belt and fell onto belt structure. EE felt pain in EE's left ribs. Diagnosed with a fractured rib.
While standing in between the engine and frame doing alternator drive accessory drive repair on CAT 793D haul truck, the employee went to step down on belly pan to get closer to work and felt like left knee popped.
Employee was reinstalling a impeller on pump and was pushing it with employee's foot to line it up so they could get it on the shaft and employee felt a popping sensation in employee's lower left back area.
While cutting a hose, knife slipped and employee cut finger.
Employee was attempting to enter the D7 dozer in the pit. As the operator reached for the top hand rail, slipped and fell backwards, landing on the ground. Employee suffered small contusions on back and right arm. Doctors put employee on restrictive duty.
A miner was injured when ee placed hand on the chuck of the roof bolter head to check for suction. EE right ring ringer was injured when a drill steel fell and contacted the tip of finger.
The Temp Contact Employee was walking down a dry dirt designated path when the employee misstepped and fell onto backside. The employee then got up, and felt a knot in right hamstring. Employee was later checked out and was diagnosed with a possible muscle strain and returned to work the following day.
The miner was feeding a new belt onto a belt conveyor when the spreader board on a chain hoist skipped free. The board flew out and struck the miner in the chest.
Employee and co-worker had cut a piece of scroll plate out, and while laying it down, the plate caught the top of employee's glove and hit their hand and wrist as they laid it down, bruising their left hand/wrist area.
Employee was operating a wheel loader, hit hole causing the seat to bottom out and jarred back. Employee went to doctor and restricted duty began on Aug 9.
EE was sweeping in the crusher basement and tripped over studs that were protruding from the ground. The EE lost their balance and in their attempts to regain their balance they strained both knees. The EE was taken for medical evaluation and placed on modified duty.
EE and two others were working in the Tunnel in the Quarry trying to raid the gate for the tail-pulley. When it became unstuck, EE's hand got stuck between the gate and the wall. The other two rushed to help free EE's hand using a piece of 2x4 wood to pry EE's hand out. It took about 3 minutes to free EE's right hand; EE's thumb was numb.
EE twisted right ankle just after getting out of vehicle. EE placed on restricted duty, 6/7/17.
Employee was repairing doors at a rock dust hole when a beam turned over and caught their right little finger, mashing and broke it. This resulted in five stitches to employee's right little finger.
Employee was operating a Skid Steer Loader and noticed the machine temp gauge was running hot and steam coming from the radiator cap. The employee turned the machine off, exited the machine, and attempted to push the cap down. The cap came off and steam burned employee's right arm.
Ee was moving a VSD on rollers. When going into the MCC, the VSD was pushed sideways into a cement wall, crushing the ee's left index finger. No fracture, but stitches required to close the wound. Ee released to restricted duty.
IE was moving an empty garbage bin into place and felt a pain in abdomen. Was diagnosed with a unilateral inguinal hernia and released to restricted duties.
Cleaning robot station and strained lower back
The employee stepped out of the Skid steer ee was being trained on to use the bathroom. EE stepped on a slick surface and hit hard hat on the bucket of the skid steer. EE had a contusion on the back of head.
A millwright was working on a pinion bearing replacement job on our raw mill and felt a pain in knee. EE does not know what caused the issue. This incident was initially reported as a first aid and later became a medical.
A Drill Helper was attempting to make a mechanical break of core to fit into the core box. As EE swung down with the hammer it glanced off the core table and changed direction causing it to strike EE's L middle finger tip between hammer and core tray resulting in a fractured finger tip.
Employee Stepped over an I-Beam, Clinker on the other side of I-beam, sprained left knee
The day following shoveling fine material, employee said they felt soreness in their back but refused a medical exam or treatment. A medical checkup was not requested until 6/27/2017 at which time they received an appointment on the 28th.
Victim was operating assigned loader. It had been raining a substantial amount. A rock fell from the highwall and struck the windshield of the loader causing dust, dirt & glass to get in EE's eyes. EMT's flushed and bandaged EE's eyes. At the hospital no foreign material was found in EE's eyes but there was minor scratches. Ointment was applied and EE was released.
At 10 am on Monday, June 5th a Cat 773B 50 ton haul truck was backed up to a Linkbelt 350 excavator. While being loaded, an air valve gave way on the truck. This caused the brakes to go out, and the driver backed into the Excavator. The haul truck and driver suffered no damage/injury. The excavator and operator suffered minor damage/minor injury.
A piece of plywood shielding board became dislodged and fell onto individuals L pinky finger resulting in a fracture.
EE says EE was doing clean up work with a shovel in the tunnel and pulled a muscle around EE's Rt rib/chest area, but then says it could have happened when EE was mowing the lawn?
EE struck in arm with loader bucket. EE placed on restricted duty for arm contusion.
Day shift production crew was heading to the section. An electrician pulled a motor into the run-around on main line 2. The employee failed to throw the switch and the on coming mantrip took the curve into the motor causing the injured employee to strain their back when the mantrip struck the idle motor.
The pin on the slot wrench broke while making a connection. The driller asked the helper to move the wrench back into place by hand. After doing so the driller placed hand back on the controls to move the hydraulic ram. Then the helper moved hand back onto the wrench as the ram moved, pinching EE's finger.
Employee was bending over to pick up a 1-1/2"" water hose to wash the loader when they felt a sharp pain in the lower back.
Day shift production crew was heading to the section. An electrician pulled a motor into the run-around on main line 2. The employee failed to throw the switch and the on coming mantrip took the curve into the motor causing the injured employee to strain their back when the mantrip stuck the idle motor.
While working on a water pump, ee alleges something stung ee on the right elbow. 6/4/2017 the elbow was swollen with pain. 6/5/2017 Dr. said it appears to be infected or fractured.
Employee injured knee when employee slipped climbing up an excavator to fuel it. This slip / injury happened on 6/2/17. Employee continued to work (after receiving 1st Aid) and worked without issue until employee woke-up on July 5th in pain. On July 5th employee had knee drained and was back to full duty on the 10th of July.
Employee slipped and fell against pallet handling device, bruising their forearm.
EE stepped onto bottom rung of ladder and felt pain in right knee. EE placed on restricted duty for knee sprain.
Cleaning a tank and upon completion the laborer-miner was pulling a shop-vac out of the tank. It became snagged on the rim and the worker attempted to pull harder to release the vacuum from the edge of the tank, it did not release and EE strained lower back.
Employee was walking up a short inclined area. The employee slipped and fell and landed on their left shoulder. Employee is on restricted duty pending an MRI.
While lacing the belt and using the belt stretcher on line four EE was holding onto the frame of the belt stretcher with right and tightening the belt using left hand and it appears there was a small notch underneath the stretcher which EE was not aware of and when pressure was applied hand slipped into the notch on the Belt Stretcher which caused the injury..
Employee stated that they walked outby to get a solid piece of curtain to hang over the fly pads between the #8 - #9 entry on 1 North. Employee noticed a piece of rock that needed scaled down and went to the bolter and got a slate bar. In the process of scaling the rock some additional rock fell out and the employee slipped landing on their left arm.
Tightening wheel on truck and ratchet slipped hitting employee in the eye causing employee to need 6 stitches.
Employee was drilling a hole in a steel pipe when the drill bit caught and caused ee to lose control of the drill. The drill rotated out of ee hand and struck ee on the face, lacerating lip. Laceration needed stitches to close.
A millwright was working outside of the maintenance shop on a hopper that the millwright was building for the CKD briquetter. While drilling a pilot hole for a liner, a 1/8"" drill bit broke. Once the 1/8"" drill bit broke, it pierced the leather gloves at the seam. The seam was between the thumb & index finger. The drill bit went into millwright's flesh about 1/4"" deep.
Employee was trying to move a garter snake from the work area when they were reportedly bitten. The incident was not the result of any compliance issues nor were any protective guards involved.
The employee was riding in a personnel carrier when the personnel carrier hit a hole in the mine floor which caused the employee to twist their neck. The employee felt pain in their back and shoulders. The employee received an injection on 1/29/18, this employee has not lost days due to this injury.
Was welding a chain on dredge and a piece of steel flew into eye.
Haul truck driver fell backward while exiting cab of truck, striking head on pavement and sustaining a 1"" cut that required staples. Employee admitted to rushing and not having mind on task, and was not certain that EE maintained 3 points of contact.
On 6/29/17, while driving a UTV an employee's gloved hand was pinched between the roll cage & the rib, resulting in a positive x-ray for the tip of finger & a laceration which required sutures. Employee was issued a full release. No days Lost.
Employee was making a splice on a roof bolter cable and struck left thumb with a hammer. Employee went to the hospital and x-rays showed no broken bones. Employee continued to work until 7-18-2017 when they returned to the doctor for a follow up visit. No new x-rays were taken, but the employee was diagnosed with a fractured left thumb based on the initial x-rays.
Employee was doing yard clean-up when the wind blew something into the left eye. Foreign object removed next day at clinic.
Operator strained muscles in the right side of neck while pulling a water hose, resulting in Physical Therapy. Employer was only notified of injury as of 8/31/17.
Employee was center bolting in F28 HG. EE was installing roof bolts in a turn out area. After drilling the hole, EE pulled out EE's steels and a rock fell from the roof and hit EE's left arm. Rock was approximately 2'x3'x3.5"". It caused a laceration to EE's left forearm which required eight sutures.
Miner was helping change worn out return idler on a conveyor. While positioning self, a small clump of mud fell from above on to miner's face and eye.
EE is a Welder Repairman was changing v-belt on conveyor while installing finger became caught between belt and sheave. EE was sent to Occupation Health for a pinch to tip of little finger Rt. Hand. Over the counter meds were given. EE stated that the tip of finger was bent inward. EE was sent to Orthopedics and a splint was placed on finger 7/25/17. Continues to work Full Duty.
Impacted right thumb when moving a screen. Was reviewed at local urgent care. Ok. No time lost.
On June 27, the employee stated right eye became irritated, the employee flushed it out, & continued on the task. Did not report the incident to supervisors. On June 28, the employee's eye became inflamed & irritated. The employee reported for work on June 29 & requested medical attention. The employee was diagnosed with a chemically burned eye & prescribed eye drops.
Employee was opening a welding rod can. The lid cut through glove causing a laceration to the second finger on EE's right hand. The laceration was closed with medical glue.
Employee got foreign object in eye when welding/chipping slag. Employee was wearing welding helmet at time of injury.
Miner/employee was drilling a piece of angle iron in the area of the Feed tunnel belt area when the hand drill being used twisted in their right hand resulting in a sprain to their right ring finger. Miner/employee did not report or seek medical attention until 07/12/2017.
EE injured left index finger when it was struck by a hammer. EE was holding a punch tool and another ee swing the hammer, missed the tool and struck the injured ee hand.
While lifting a hydraulic wrench, employee strained right shoulder.
Employee was off loading used belt rollers from a supply car into the bucket of an end loader. The end loader bucket contained some loose trash. One of the rollers contacted a 4' mine bolt in the bucket causing it to spring upward striking the employee in the upper lip, lacerating the lip and chipping a tooth.
Employee was using a grinder when employee felt debris go under employee's safety glasses. Once the employee went home employee noticed a piece of metal was in employee's right eye. Employee went to the doctor where they removed the metal and gave employee eye drops.
While making a belt splice, the injured miner was using fingers to feel the nail heads to ensure they were adequately seated in the splice while the other miner was still hammering. The injured miner's finger was struck by the hammer resulting in a fracture.
Employee was helping place a cutting edge into the back of a pickup. The cutting edge slipped and smashed their finger between the cutting edge and the tail gate.
Mechanic was lowering an extension ladder 135 inches to the ground from a safety railed platform with toe boards. The ladder began to slide down the rail when the bracket for keeping the extension ladder in one piece slid against left index finger. Mechanic was not wearing gloves during the incident. The wound required 8 stitches to close. There are no restrictions on work duty.
EE was laying on the screening media when working. Upon sitting up, ee felt a sharp stabbing pain in lower right abdomen. EE was replacing both the upper and lower motors on pre wash.
Miner was using a sledgehammer to breakup debris. Felt something pop in their R shoulder area.
Stepped wrong on the bed of the truck while stacking cribbing, started to fall and caught self twisting right knee
Employee was drilling a 5/8"" holes in 3""x3"" angle iron with a Wilton drill press. The piece pulled up out of the vice and grabbed EE's welding glove on EE's left hand. The drill twisted EE's hand and arm into the bit until it pulled off the glove and the bit broke. This accident resulted in a hairline distal fracture of the left ulna.
Employee was stung on the face twice by a wasp.
REPORTABLE ONLY - Employee was chipping welding flux from a headgate cowl, when a piece of the flux came up under the safety glasses scratching employees eye.
Employee was pushing up a roof bolt with the roof bolting machine and as employee was guiding the roof bolt to the mine roof the plate caught the drill steel guide causing the plate to cock pinching employee's left thumb resulting in a laceration.
EE was taking a 1 1/2"" water line loose at the pump when water line sprayed EE in the face and some debris got into eye under the safety glasses. EE went to ER initially, followed up with the eye doctor. EE had piece of metal in the right eye and it was removed. EE has no lost time. EE has returned to work.
EE was removing an oil filter from a track mobile engine and later that day ee felt pain in left elbow. This was considered a first aid case however on 08/02/2017 ee received a medical procedure that made this incident MSHA reportable.
Employee was a part of a crew that was assembling portion of a conveyor chain. To assemble the chain, pins must be pushed out hydraulically with a pin pusher. Employee felt pain in left shoulder after lifting pin pusher. Injury became reportable on 8/9/2017.
The L8 drill operator was exiting the cab of the drill when EE pinched and cut the pinkie finger of right hand on the door grab bar and latch. The operator had removed the leather utility gloves was wearing a pair of light nitrile gloves as EE was going spray paint markers for drill holes.
While servicing HVAC outdoor unit tripped on refrigerant hoses with the top of unit off and running. Fell into unit with left arm causing a laceration to the left wrist against the blower motor fan blade.
An employee received a laceration while tightening a bolt with a hammer wrench that slipped, pinching employee's finger between the wrench and breather pipe cap. The employee was treated on site before being taken to the local hospital where the wound was closed with seven stitches. The employee was released back to work without restriction.
While re-stacking hydraulic cylinders from wooden pallet to plastic pallet, employee pinched finger between two cylinders when they shifted.
While pushing a pallet of garnet with a pallet jack, an employee felt a pop in the left foot that was followed by a sharp pain. The employee received initial diagnostics, but did not receive treatment until 2018.
The employee was walking an tripped on a small rock. The employee fell hitting their knee on the mine floor.
Instrumentation Tech was removing leak detector pump #23 at Underdrainage #3 when the employee experienced issues removing leak detector from pipe - getting stuck and slipping. The employee then put on latex/vinyl gloves when attempting to remove it again the pump slipped and lacerated the right index finger at the joint closest to the palm.
Helping a mechanic install a bottom roller on a dozer. The roller set down onto the hand creating the laceration.
Employee was using a pry bar to pry class B material (erosion control stone) loose in a clogged chute. As the employee pulled on the pry bar, it slipped loose striking the employee in the left eyebrow area.
A maintenance mechanic was using rigging to move raw mill shell liners. The employees right pinky finger was pinched between the shackle and magnet that was being used to move the liners. This resulted in a fracture on the tip of the pinky finger.
Employee sliding electrical control panel when it shifted and pinched right pinky finger between control panel and concrete steps requiring sutures. Employee was released to return to work.
EE was moving some metal screens around in the back of pickup truck with the help of a vendor. Doing so, EE's finger got caught between two of them and caused the thin edge of the screen to cut into finger tip. It cut part of the nail so it required 2 stitches.
Employee was stripping wood frame around A/C unit and bending nails over as they were pulled. During clean up, EE stepped on a nail that was apparently missed. The nail had penetrated the work boot and punctured the bottom of foot. Foot was cleaned, received a tetanus shot, and was given precautionary prescription. No lost time.
Employee was operating continuous miner cutting in 1 Left when a piece of draw rock fell and hit EE on top of right hand. 14 stitches.
Prepping plant for production. Released a discharge gate and caught hand in gate. Cut/Abrasion to hand resulting in 3 stitches.
EE was on level 3 of the tower. Pallets of material was being lifted up to this level by a crane. EE was using a tag line to help guide the load over a handrail when ee felt a pain in right bicep. EE returned to job but on 7/20/17 during a follow up visit the Dr. Prescribed physical therapy.
Employee stepped out of their truck on to a piece of dirt and twisted their ankle
While cleaning the pre-heater tower at the level 1A feed shelf, and wearing the proper personal protective equipment, a large plume of dust came up and under EE's face shield and safety glasses and got into EE's right eye. The tower went slightly positive. The injury is a corneal abrasion of the right eye.
Employee was installing kiln trunnion bearing, while ee working on the bearing pedestal platform. As ee stepped down from the platform, right knee made contact with a piece of angle iron attached to the platform, causing bruise. Treated with OTC meds until Xrays/MRI taken. Ortho prescribed anti inflammatory and phys therapy on 9/5/17.

Contractor pushed left thumb into a wire wheel on a pedestal grinder while cleaning bolts. The flesh was removed from thumb.
EE was spreading belt structure in belt line when ee tripped and fell into the belt structure bruising right rib cage. The employee continues to work. On 7/9/17 the employee went to the Doctor and was diagnosed with a fractured rib. This is a Medical Treatment accident.
Employee lost control of vehicle while descending the ramp. EE hit a rock which stopped the vehicle. EE left hand hit dashboard on impact.
Repair work on crusher, trying to loosen nuts with a ¾ ratchet and a 4 ft pipe and strained their back.
Injured employee sustained a laceration to head, requiring sutures to close, after the motor EE was operating derailed on Ease Main Track.
was rolling peace of thin metal around another with heat and come-a-long. chain popped on come-a-long and hit employee in face. causing a cple lacerations on face and nose.
Was using a grinder on tailgate and the grinder got caught and jerked back into nose. Nostril was cut and received sutures at Salem Clinic Urgent Care.
Employee was closing a water gate valve when their nose came in contact with the wheel on the gate valve.
Employee was operating pipe fusing machine. EE placed EE's hand on the hydraulic clamp to readjust. At the same time, using EE's other hand, EE operated the controls to close the clamp. The inside of EE's thumb was pinched between the pipe clamp and clamp support.
When putting top rollers in a scoop bucket, one stand kicked up and hit EE's eye. EE received 4 stitches in eyebrow. No work missed.
Employee reported on 6/25/17 that EE injured arm 6/16/17. EE was removing a seat from a Euclid hauler using a wrench and the back right bolt was very tight. The DR did not give any restrictions on initial visit. EE eventually required physical therapy on 8/22/17 which makes this a recordable injury.
Employee received a laceration of the right forearm when a piece of banding steel that was being removed from an item sprung back and struck the employees forearm.
Mechanic was installing the right tracks on a dozer, employee was using a bar to align the bolts when the bar slipped and pinched employee's finger on employee's right hand resulting in a laceration to employee's ring finger which required 4 stitches. The employee was released to return to work and did not miss any scheduled shifts.
While exiting the 62 shovel, employee realized that ee forgot to turn on the MG set. The employee turned and caught the door latch as it was closing. EE left index finger got pinched between the door jam resulting in a laceration and fracture to the tip of ee left index finger.
The miner was lowering and moving a vertical Kennedy Stopping Panel when a small piece of top fell striking employee on the right side of employee's face (employee's cheek) beside of employee's nose.
Employee utilized dual eye protection while grinding overhead on the 51 shovel project. At shift end, the employee felt something in right eye but did not see anything. The employee reported to the eye doctor the next morning. A small piece of metal was removed and the employee received prescription eye drops.
The employee was taking a nut off the head of the crusher when the nut fell hitting EE in the right forearm below the elbow causing a cut requiring 8 stitches.
Reportable only due to sutures. Employee was using a grinder to cut some bolts off when the grinder slipped and cut EE's left index finger.
Employee was pulling on gate to change product flow in crusher finishing tower, put left hand in inappropriate area causing pinching laceration on left hand, ring/middle finger.
While splice a cable the employee cut through protective gloves and caused a laceration to right thumb. 3 sutures were used to close the wound.
Injured employee was loading a roof bolter with supplies. The employee was putting a handful of 8' bolts (~4 bolts) onto the rear of the bolter. The bolts contacted a bundle of roof bolt plates causing them to fall from the bolter. The bundle of plates struck the employee's right arm causing a laceration that required 3 stitches.
Employee was filling up duster at fan. Employee had right hand sitting on top of the dust hopper on the fan when a small piece of loose rock came off of corner between roof and rib striking employee's hand, causing employee to receive stitches
Employee stated EE was dropping bolter cable, the cable had tension which caused the cable to drop fast and caused hand to come in contact with a monkey face causing a laceration to EE's right hand. Employee received stitches.
