At 7:55 am on 12/31/2010, the elevator at the Hart Street Portal had the door pulley break. The secondary escapeway was rerouted to the South Portal and repairs were made. The elevator was back in service at 10:50am

A contractor was stuck in the Pellet plant elevator for approx. 1 hour and 40 minutes. MSHA was notified. No injury.
Fall was found at 45 break on 1st left off of south mains, 2 breaks inby spad # 1662. Fall was 30' long x 19' wide x 5' to 6' thick. Roof support was 6' torque tension bolts.
Employee was operating a D-11 Cat dozer ,developed a hydraulic leak , called for maintenance, traveled 200' the dozer had caught fire. Tried to put out the fire could not. The foreman called the fire department, the fire was out at 8:25 p.m. .
There has been a fall found one break outby spad no. 1304 in the no.1 entry. The fall measured 22'long x 5' high x 20 wide. This fall has been cleaned up and the area rehabilitated.
A unintentional fall of the roof on #1 unit's 1st beltline at xc 25.
ROOF FALL: A roof fall was found by #2 LW crew as they were cutting coal on the face from the H/G to the T/G. It is located in an intersection of T/G entry Spad 21396. It fell above the anchorage of the primary roof support due to extreme pressure of the gob and a vertical degas well. It was about 20' long X 25' wide X 7' high at the highest point.
An unintentional roof fall occurred on 12-2-10. The fall occurred on old 48 belt, #18 crosscut, #4 entry (spade #2324) to #2 entry (spade #2318. 2 crosscuts our.) No injuries occurred from the fall. Length 140' X width 20"" X thickness average average. Support 6"" fully grouted bolts.
A roof fall was discovered in the 3rd Northwest submains xc-39 between #3 and #4 entry spad #29+10. The approximate size of the fall was length - 40ft, width -18ft and height 6ft plus.
There has been an unplanned roof fall in the no.2 entry 2 breaks outby spad no. 1303. The fall measured 35' long x 5' high x 19' wide. This fall is located approx 14,000' from the working section. All access entrys to this fall has been timbered and dangered off.
Wet slicky sided slip formation - Unintentional roof fall occurred 8,600 ft from 001 MMU. Fall measured 40' long by 18' wide by 8' thick. Fall will not be cleaned up. This fall does not affect ventilation or prevent us from doing out weekly examinations.
Slope car was down due to changing out magnetic switch.
A fall of the roof occurred at crosscut 93 in the limited intake in the South East Life of Mine Mains B in the # 7 Entry. The ventilation, personnel nor equipment were affected.
Area was already bolted with 6' tension bolts and 12' cable bolts every 8' fall occured in the #4 entry at 37 block on the 7 North Submain area. Cribs set on the inby and outby corners and crosscut 4-3 had been center cribbed before the intersection fell in.
The elevator has been recently upgraded. An employee attempted to complete a PM of the elevator in the normal manner. With the recent upgrades this method did not work (the installer of the upgrades did not notice this issue). When he did the normal procedure, the elevator became inoperable.
Foreman at RB#5 Mine reported a roof fall, he said it was in the #3 entry inside the mine from the back side. The fall measured approx. 20' X 20' X 4' thick. There was no one injured. He said it fell sometime between the morning and that afternoon.
An unplanned roof fall has occurred approximately 1400 feet outby #2 Section at spad #3535. The fall was 20ft x 25ft x 8ft. No injuries or equipment was involved.
The limit switch on the elevator door was not working properly. This allowed the elevator to be inoperable for a period of time.
An unintentional roof fall has occurred at the #41 xc of the 2nd Main South between the entries of #3 and #2. The fall was approximately 55 feet in length and 17 feet wide and approximately 8 feet high.
Elevator went down with two miners onboard. Ice accumulated on the bottom and would not allow elevator to operate. Personnel were trapped for 41 minutes.
Our Highwall Miner hit an unmapped/ Unknown abandoned mine which was full of water. The water started to fill the pit area that we were in so we retracted our miner a pulled out of the pit area.
The plant operator stopped the plant feed belt to make a repair. The operator & another ee, the plant runner forgot to turn off a torch on the plant feed belt that he left unattended. The belt caught fire and was put out in less than 10 min. 2 ee went to the hosp to be checked out for smoke inhalation and returned to work in a few hours.
16' Production Shaft - Accident only, skip came into contact with the divider beam resulting in damage to guides, divider beams and tail rope
A roof fall occurred on 002 section in #2 entry in the intersection just inby spad 316. The fall did not affect persons, ventilation nor equipment. The fall was appx. 30'L x 20'W x 9'H. The fall will be cleaned up and resupported with roof bolts.
An unplanned roof fall occurred in the 1st left off old #11 belt 3 blks inby spad # 710 in the old face area. This area was developed on 12-15-06 with approximately 160' of cover and the roof is supported with 6'fully grouted resin bolts.
A roof fall was discovered at 1 south break #46 between the #3 & 4 entries measuring approximately 20'W X 6'T X 30'L.
Employee was in the freight elevator on his way to the 4th floor when the elevator stopped.
A roof fall occured on the 1st.54 supply road at 19+20-20+00 #24-#25 x-cut measuring approx. 18'x14.5'x6' caused by an undetected slip in the mine roof. The area had been mined in Aug. 2005 and supported by 6' tension re-bar roof bolts and cable trusses.
A roof fall was discovered at 3 headgate break #3 between the #2 and #3 entries measuring approximately 20'W X 6'T X 80'L. This was originally reported to MSHA on form 7000-1 on 12/23/10.
A roof fall was found at break 35 on the # 9 beltline, it was approximately 40' by 20' 10' thick.
Rockfall was discovered on 12/21/10 in the 1st right mains #4 entry on the intake neutral. Rock was approximately 7'-8' high, 50' long and 19' wide. No ventilation was disturbed , no injuries, no equipment was involved, fall has been timbered and dangered off.
An unintentional roof fall occurred at 88+53 on the 2nd West between the belt entry and the travel-way. The fall did not affect ventilation, affect passage of men or cause person injuries. The fall measured 45' x 19' x 6' high. The fall will not be cleaned up. The crosscut had been bolted with 4 foot fully grouted roof bolts.

A fall occurred on the # 7 belt 1 break inby spad 7977 of the B Panel of 2 Southwest Mains that was 5' thick , 16' wide, and 55' long. Area was dangered off until an approved cleanup plan could be acquired through MSHA. Area was cleaned and re-bolted.
The hoist car serving the #1 track slope was inoperable due to the brakes being locked up. The car was down from 9:30 am until 4:16 pm and was put back into service when the brakes were repaired.
An unintentional roof fall occurred on the 4G at station 48+30 in the crosscut between the belt and travel way. The fall measured 18feet x 18feet x 6feet. The fall did not affect ventilation, passage of men or cause personal injury. The fall did cover up a 13 h.p. water pump.
Slope car was down due to electrical problems.
Upon an investigation the D-8R stockpile dozer was shoving coal into No.1 feeder and when backing around stacker the machine went down into small void at feeder No.2. To prevent a re-occurrence spot lights were installed, upon dozer operator's request, for visibility when operating at and near feeders, and changing operating procedures to prohibit backing passing feeders.
A roof fall occurred on #3 unit, spad 8+25. The fall is approximately 19 feet wide, 12 feet high and 76 feet long.
A continuation of a fall that was reported on 8/25/2008 was discovered at 11:30AM on 12/17/10. The fall was located between spad #133 and #134 between #2 and #3 entries on the #1 Southwest Panel at break 19. The fall measured approximately 18' wide x 60' long x 7'-8' high.
RECEIVED A CALL OF WATER LEAVING THE PROPERTY, UPON INVESTIGATING THE COMPLAINT AN APPROXIMATELY 12'X10' AREA OF A TAILINGS POND DAM WAS FOUND TO BE BREACHED.
A roof fall was discovered in the 1st North Mains #5 entry, between spad #3 + 80 and #4 + 50 and xc-6 and xc-7. The size of the fall was approximately length - 30ft, width - 16ft and height - 6ft plus.
4 left panel off of 1 west mains, #9 entry, 7 break. Dimensions were 35' long x 6' to 7' thick x 18.5' wide. Roof control was 6' torque tension bolts.
The compressor to the hoist went down, causing the hoist to not be usable for about half an hour. We started the back up compressor until we could get the normal compressor going. That took about 40 minutes.
A roof fall occured 2 breaks inby spad 1360. The fall was aprox. 10 feet high X 20 feet wide and 20 feet long. The fall area has been cribbed and dangered off. The fall does not affect any travelways and airways. Fed. inspector has been to the fall area and inspected the area on 12/15/10.
An unplanned roof fall occurred in the #4 entry at 1 break outby spad #2200. Fall measured 20'long X 20'wide X 6'thick. No equipment or injuries. Roof fall did not affect ventilation. Roof fall did affect passage of persons due to being in track entry. There is no active section at the location fall will be rehabilitated. Federal and State notified immediately.
A roof fall was discovered in the old 1 East panel in break #7 near spad 1308. Breaker posts were set to prevent travel to the fall. Travel and ventilation was not effected by the fall.
A non-injury accident, the unintentional stopping of the Camp Run Portal elevator. The source of the malfunction was found to be with the top door hatch switch. The elevator was returned to service at approximately 12:54 PM. Schindler Elevator Service was call to evaulate the condition.
Fall occurred on 12/14/10 found at approximately 6:45 am by Anthony Miller. Fall occurred on 1 West Mains in the #3 entry 2 cross cuts inby spad station 5605 and extended to spad # 3767. Fall was 100 ft long, 6 ft high, 20 ft wide. Fall did not alter the ventilation or affect the travel way. This fall was timbered off and danger signs were posted to restrict access to this area.
An unplanned roof fall occurred in the return at break #28 in the #1 entry along the #2 beltline 1 break inby spad #261.
We had an unintentional roof fall in 3rd West. It was in #4 entry and inby XC 24. The area is not a working section. It did not affect ventilation or involve an escapeway.
Fall was found 1st left off of 1 west mains break 28, track entry spad # 568. Dimensions were 12' wide x 25' long x 6' thick. Roof support was 5' torque tension bolts.
A roof fall was discovered at No.19 Crosscut on No. 3 belt in the belt entry. The fall measured approximately 20'x 20'x 8'. There were no injuries from this occurrence.
A roof fall measuring 20'x20'x6' occurred near spad number 1691 near old 2 left panel. This area is approximately 8000 feet outby face and the fall area will not be cleaned up.
A ROOF FALL OCCURRED IN THE NO. 1 ENTRY TWO BREAKS INBY SURVEY STATION NO. 26118 OF THE 8TH NORTHWEST PANEL BLEEDER SYSTEM. THE FALL IS APPROXIMATELY 18 FEET LONG X 18 FEET WIDE X 10 FEET THICK. ROOF FALL WAS FOUND AND REPORTED BY MINE FOREMAN AND OMSL INSPECTOR WHILE TRAVELING THE BLEEDER SYSTEM.
At approximately 11:55 the elevator became in operable. After investigation was determined that a control cable had been severed. The cable was repaired immediately.
An unintentional roof fall occurred on the #4 belt in the #4 entry 30 feet inby spad #1025between the blocks. estamated size of fall 15'L. X 18' W. X 4-5' H. Area had been supported with 4' fully grouted resin bolts. No injuries occurred, ventilation was not interrupted, conveyor belt was caught. Fall will be cleaned up.
Discovered that the roof had come in around the cribs. The fall occurred between station 22+37 and extend to 22+20. A location just inby x-cut 12.
Contractors on site to remove unused equipment, were cutting iron with torches to remove entrance to building. Contractors inspected area an hour after cutting for embers, saw none and left the area at roughly 5 pm. Our employees noticed fire at 10:45 pm and called the VFD and me. VFD arrived at roughly 11 pm and contained fire to Old MCC room and extinguished.
Pleasant Grove portal elevator would not move from top landing due to doors not closing with door switch. The switch was cleaned by mechanics and operations resumed at 6:55 A.M.
A hoisting accident occurred at the Contrary Portal. A large piece of ice fell from the shaft wall striking the personnel hoist cage. The ice caused the canopy to shift off of it's supports. The canopy got caught on the landing when it was brought to the top of the shaft.
#9 Entry - intake (outby) Roof Fall 17' wide x 80' long x 6-8' thick
Mining in area of laminated top and stack rock. When brk was cut through area of unsupported top fell pulling two rows of bolts in the #3 entry. Test holes in all out-by and face areas did not show signs of fall area. Area was cribbed and dangered off to prevent any more mining. There were no injuries.
The mine examiner found a roof fall above the anchorage near the # 2conveyor belt at survey point #333.
A roof fall occurred on the 17w section at 12:20pm at 76 block inby the belt tail pieces. No injuries were reported. Ventilation to the #1 face was obstructed and corrected. The fall will be bolted with rod bolts and cleaned up. A joy shuttle car is trapped under brow fall and will be removed.
During pillaring a planned fall occurred, the pressure from this fall effected the ventilation by blowing out the brattice line, production was stopped, workers brought out, and necessary steps were taken to restore ventilation. Ventilation has been restored.
A faulty over travel switch on the Hart Street elevator was discovered during an inspection by MSHA. The elevator was immediately taken out of service and repairs made.
#2 hoist faulted and ""kicked"" out. The hoist was reset and an investigation found that the cooling fan in the hoist breaker box needed replaced. There were still two means of escape to surface available for personnel underground.
Spontanious combustion occured in coal being stored at the No. 1 upper stockpile/blending area. Dirt was used to cover and extinguish the flames. This action was performed within approx. 15 to 20 minutes of discovery.
A unintentional roof fall occurred in the 4G intake in the #3 entry at 12+30. The fall measured 30 feet x 18 feet x 6 feet high. It did not affect ventilation, passage of men or cause personal injury. The area had been roof bolted with fully grouted 6 foot roof bolts.
A roof fall occured on the 2nd West Main, spad 0+80 in the supply road. The fall is 20'x20' height unknown.
A ROCK FALL WAS FOUND IN THE #3 ENTRY OF SMOOT CRK. MAINS ONE OF THE RETURN ENTRIES. THE FALL WAS APPROXIMATELY 18 FEET WIDE 40 FEET LONG AND 10 FEET HIGH. THERE ARE 2 ADDITIONAL RETURN ENTRIES IN THIS AREA.
A fall of roof occurred in the South East Submain at 33 cross cut, # 7 Entry. The fall measured approximately 20 feet long x 20 feet wide and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
A unintentional roof fall occurred in the 4G return in the #1 entry at 12+30. The fall measured 25 feet x 20 feet x 4 feet high. It did not affect ventilation, passage of men or cause personal injury. The area had been roof bolted with fully grouted 4 foot roof bolts.
A roof fall was found off 8 track at brk. 31 in #5 entry, extends thru #2 entry to the right, the size of the fall is approx. 60' long x 20' wide x 8'to 10' high. No ventilation, or escapeway affected. The area has been cribbed and dangered off.
Due to inclement weather, the door switches at the bottom of the elevator were not operating properly.
An unintentional fall occurred in the main north #10 entry at the 17+30'. The fall was 19' X 19' X 10' in the return aircourse.
Fall was found on 4 left panel off of 1 west mains, between 4 break and 6 break #9 entry. Spad #1521 in #8 entry across from fall. Dimensions were 60' long x 7' to 8' thick x 19' wide. Roof support was 6' torque tension bolts.

During a weekley exam of the intake split of air(not the primary escapeway) used to ventilate the # 3 working section a roof fall was encountered. It was 50' long and 18' wide. All approaches were cribbed and dangered off.

An unplanned roof fall has occurred at #4A Belt line break 23 on #2 Section inby spad #6011. The roof fall was 20' x 20' x 10'. No injuries or equipment was involved.
A roof fall occurred, in the #8 Entry, in the last open crosscut on a development area for the #1 Section. The fall occurred in an intersection of the 3rd East Mains at spad # +4080. The primary roof support for the intersection was 6' fully grouted bolts with supplemental 8' Tension Rebar bolts.
Pickup truck and coal truck hit each other - No injuries.
Roof Fall occured at the airshaft mains area of the mine in the No. 5 intake entry at crosscut No. 9 spad No. 4167. The roof fall is approximately 12 ft in height, by 18 ft wide, by 22 ft in length is throughout the entire intersection.
Roof Fall in the brushy mains between survey stations 14081 & 14085 in between #1 and #2 entries. MSHA inspector found this fall during his examination.
Drum brake locked up on hoist due to interlock being frozen.
The hoist car a 20 ton motor and a rail car full of rails was being lowered into the track slope. The rails came off the inby end of the car and caused the hoist car to de-rail. The slope car was inoperable until 2:00pm Sunday, Dec 5, 2010.
A ROOF FALL MEASURING 70'(L) X 18'(W) X 6'(T) WAS DISCOVERED 80' INBY SPAD #1293. THIS FALL WAS DISCOVERED @ 10:20AM.
A roof fall occurred in the 0 Entry of the return between spad +3760 & + 3840. (Between crosscuts 47 & 48) of the 2nd Southwest Mains. Dimentions are 40' (L), 18.5'(W), & 8.5'(H). Primary Roof Support in the area was 6' Fully Grouted Bolts with 8' Tension Rebar as supplemental support.
An unintentional roof fall occurred at break 65 between #5 and #6 entries (between spad 370 and 375). The fall measured approximately 20 ft wide x 30 ft long x 5 ft high.
A fall of the roof occurred in the South East Life of Mine Mains C at crosscut 442 on the 10th 54 in the #4 entry. No ventilation, equipment nor personnel were affected.
An unplanned roof fall occured in the 13 I longwall section in the belt entry @ 27+40 and extended to 27+90. No injurys or equipment damage as a result of this incident.
A roof fall occurred in the 1st West Panel off the Main North in 1/2 the intersections of #5 and #6 entries at spad number 27+90 and extending through 1/2 the intersections of spad number 28+40 measuring approximately 70ft. long x 19ft. wide x 8ft. high. The area was supported with 4 foot fully grouted roof bolts and 8 foot cable bolts.
The initial cave behind the shields after a longwall move has been completed, is expected after mining the first 100-feet. The initial cave for the 17-left Longwall occurred on December 3, 2010 at 5:53am. MSHA was contacted immediately, which resulted in MSHA placing a 103-J order on the mine until ventilation was restored in 17-Left longwall.
During the Accident Investigation, a team encountered a roof fall at Spad 19443 in #6 entry of the Old North Mains. The approximate size is 18' wide x 20' long x 6' high. Ventilation/travel was not impaired. The fall will be breakered off.

A fall of roof occurred in South West Submain at cross cut 38, entry 3-4. The fall measured 20 feet long x 18 feet wide x 6 feet in depth. The area was supported with 6 foot fully grouted roof bolts.
An unintentional roof fall occured on the above date at break 23 in the return. The fall did not impede travel nor ventilation and no one was injured. The fall measured 40ft. long, 19ft wide and 6ft heigh.
A roof fall was discovered @ spad #18 @ 2:10pm on 12/1/10. It measured 12'(L) x 19'(W) x 0 to 5""(T). It doesn't impede ventilation or travel and has been cribbed & dangered off.
Unplanned roof fall was discovered at 47 crosscut along 17 belt in the neutral airway entry. Fall was approximately 20 ft length X 20 ft wide X 4-6 ft in height. No injuries, equipment, ventilation or passageway affected. Area developed 7/08. Fall will dangered off and crib/timbers set around the fall.
Rock fall occurred on 1st left off 1 West Mains intake #4 entry. Fall measured approximately 20' wide x 20' long x 6' high.
Employee was changing right lift cylinder on the Gehl skid steer loader. Employee is believed to have been entering the cab when the fork dropped to the ground. The employee had his head caught between the frame of the skid steer and the arm to raise the forks.
The senior blaster received fatal injuries when he was conducting a post-blast examination. The accident occurred when the ground on which he was walking caved in. The circumstances surrounding the accident and the cause of the accident are still under investigation.
4 workers were taking screens out of tower early in the shift. Later, supervisor looked & saw no one so he turned the plant on. Victim was standing on a belt conveyor, working inside the chute & the belt conveyor started. He was pulled out of the chute & conveyed under 2 other chutes located on the same belt conveyor. After the belt was shut down he was found under 3rd chute.
Employee had a 2"" high pressure water line blow out. Pressure of the water pushed him over handrail, causing him to fall 34 feet. Employee landed on an electrical tray, then rolled off, falling to the ground. **EE was hospitalized multiple times due to his injuries and he died on July 26, 2012**
Investigation is still going on at this time. Per preliminary report: Descending the coal haul road, victim began to lose brakes. He attempted to straddle an axle berm on his left. The truck ran up onto the berm for approximately 100' before it turned over. Victim was found partly out of the passenger side door beneath the right exaust stack. Unable to remove him until EMS arrived.
EE was working on a crusher cone removing pistons from the cylinder. After removing 4 pistons, EE started to remove the 5th one when it shot out and struck EE in the right index finger.
Maintenance employee was trying to un-clog a product line beneath Dust Collector/Rotary Valve. Employee accessed 6"" cap beneath rotary valve and stuck his hand into line and into rotary valve. Rotary valve was not properly locked/tagged out. Injury resulted in partial amputation of three fingers. Employee was transported/treated at local ER, and released.
Member was swinging drill boom in when he started to fall, he was trying to catch himself by grabbing the top of the canopy with his left hand. Member continued to fall into the control levers and raised canopy into roof causing lacerations to his middle finger, which lead to doctors amputating the tip of his middle finger.
While checking a screw conveyor, the motor started unexpectedly, causing injury to an employees hands.
During the clean out of a mixing machine, access door to machine was open to allow mortar to flow out. Paddle was positioned to prevent the flow of mortar. While one person instructed the oper ator to jog the machine the injured reached into door to remove rock. Injury resulted in fracture and laceration ultimately leading to an amputation of the finger above the large knuckle.
EE was uncoupling a mine car from a motor. The jenny couples were connected with a chain. EE had removed the chain and signaled to other motorman to pull off. When pulling the car away from the motor, the EE's right hand was pinched between the chain and jenny couple. EE suffered 3 fractured fingers and amputation to the first distal of the ring finger.
Employee was installing roof bolts when he had his right thumb caught between the drill chuck and the mine roof. The employee received a laceration.
The roof bolt crew moved from #7 to #6 when the bolts in the tray snagged the line curtain and began pulling supplies off. The off side operator began freeing the curtain while the operator trammed forward to give slack in the curtain. The bolt machine tracks rolled over the tip of the employees right foot partially amputating the tip of his big toe.
Employee was operating haul truck 425 and was being loaded by the ES17 shovel. The shovel operator lost power to the bucket controls and the bucket swung into the back of the haul truck jarring the HT operator. He was evaluated and released and instructed to return to work from hospital.Did not report to work 1/1/11 and was sent back out for treatment on 1/3/ neck strain.
Fueler was removing fueling hose on a 992 loader when he felt a sharp pain in his left calf and felt pressure on it. He fell to the ground for a minute and then continued to work.

Walking down slight ramp at bottom of stairs in office building. Slipped on floor and fell.
Walking in underground construction shanty and stepped off a ledge, twisting right ankle. The strain resulted in a dislocation that required surgery. He began missing time his next regularly scheduled shift on 12/31/10.
EE said he was taking bolt machine cable down, that was hung from top when the bolt machine operator engaged the cable reel causing the cable to jerk, he jumped out of the way, twisting his knee.
Employee was pulling cables. He backed into a roof strap and cut back.
Employee was operating a Continuous Miner in Unit #3. He was turning a xcut to the left standing directly under a roof bolt. A rock approximately 2ft. x 4ft. x 1in. to 1ft. fell and struck him on the left side of his head, neck, shoulder and leg. He received a puncture wound to the left side of his head behind his left ear that required 5 staples to close.
Employee was performing maintenance near the silo at the area known as Rt. 309 Water Treatment. Employee went to step with his right foot near the edge of the water discharge stream and his right foot sunk in causing employee to lose his balance and fall to his knee(s). Employee's 17"" muck boots filled with treated lime water causing burns on his feet.
Stated: While reclaiming belt rollers (48"" top), was pitching roller on top of pile and felt a pop in his right shoulder. He had been loading rollers for approximately two hours.
EE stated that while working near the #3 belt head he slipped on a piece of belt and twisted his ankle.
Received report from Employee's doctor on 5/17/2011 that, as a result of a slip and fall on December 29, 2010 while clocking in on the company time clock, the employee has been placed on disability.
EE was examining a power station on #2 portal bottom area. He was coming through a mandoor and strained his rt knee. Began losing time on 1/4/11.
Employee stated that while working on boom spreader chain he got his fingers caught between the chain and the bed of the lizard truck.
The injured was driving on the Bearpen haulroad and as he started to negotiate a 3-4 percent grade and curve at road marker #18, his pickup truck slid into a loaded coal truck. Due to the potential serious injuries sustained to his head and chest areas, he was air lifted to a local hospital; he was released on 12/31/2010. The coal truck driver sustained first aid injuries.
Wrecking forms away from concrete, crow bar slipped and just had a light sprain to left hand wrist.
EE got his right arm caught in the man door and jammed it into rib.
Employee was crawling under a dust collector pipe on a silo when he felt his knee pop. This was originally a report only, the employee did not seek medical attention until January 4, 2011.
Employee was climbing off the drill at the end of the shift. His grippers caught the bottom step and he landed on the side of his ankle. He was sent to the hospital. Accident resulted in a fractured right distal fibula.
Employee was changing bearings on a crumbler roller when right hand was pinched/slammed and received approx. a 3"" laceration to right palm which required stitches.
Employee was rebuilding a cleaner. He tried to adjust pipe stand. When he pulled up on the release lever, he felt his left shoulder pop followed by pain. Treated with first aid. Employee was sent out for treatment after he reported having more pain on 1/11/2011. Left shoulder strain.
Member was walking beside #4 drive motors when his left foot became stuck in mud causing him to twist his knee. Member ceased work immediately and drove himself to the doctor.

Employee tripped over a rock and fell striking his shoulder on the mine floor. Employee started missing 03-14-11.
Employee was preparing to remove a large piece of metal from a compressor room and removed the safety bar across the back of the door in order to open the door and removed the metal. The employee proceeded to step through the doorway without recognizing the 2 foot drop to ground level and fell through the opening landing on the ground below on his right side.
Employee was helping to make a belt splice, when he knuckled the clamp off the crossbar came back and struck employee on the head.
EE lifted a 100 pound bag with the help of his left leg when he experienced a pull in his muscle followed by some pain. He went to doctor and took a few days off to heal. He was diagnosed with a pulled groin muscle and was released to work on 01/04/2010.
Employee stated that on 12/28/2010 while getting on the dozer, he slipped on the icy track and twisted his back. Employee did not miss work until 2/14/2011.
EE was stepping off the miner, when he stepped on a piece of rock, twisting his right ankle. EE continued to work until 1/4/11 when he could no longer work with the pain.
Employee was moving an extension ladder and pulled a muscle in his lower back.
EE was attempting to climb onto the 850 John Deere dozer. He had placed one foot on the step. When he tried to place the 2nd foot on the step, that foot slipped in the snow. He was holding onto the handrail. When his foot slipped, it caused him to twist and fall against the tracks of the dozer, pulling a muscle and pinching a nerve in his back.

The injured person was assisting with repairs on the crushing plant. He was helping to remove and carry a bearing for a shaker. The injury was not noticed at the time. He reported a sore back when he reported for his work shift on 12/30/10.
Employee was dismounting his dozer, his foot slipped on the lower push arm step, causing him to fall into the push arm with his torso.
Employee stated he was roof bolting, dropped his steels from the roof and reached for them with his left hand while his right hand was still on the control lever. He stated the motion of reaching caused him to push the lever, pinning his left ring finger between the steels.
Employee stepped off edge of concrete block at fuel pump and twisted left knee.
EE was working normally when he bent down to pick up a tool and felt pain in his lower back. Diagnosed as possible lumbago.
Member stated he used forklift to try and pick up a piece of belt and turned sharp, causing the forklift to pivot up in the middle and slam his head into the top. He felt a pop in his neck on the left side. The area was 48"" high and 18 1/2 feet wide. Member was taken by ambulance to the hospital where he was diagnosed with a fractured vertebrae.
Driver drove to the dump off pile, got out of his truck wearing PPE but no gloves, walked to the rear of the truck, opened the tailgate of the trailer with his hands & tried to get 200 lbs. of sand out of the trailer bed. When driver was attempting to ensure sand was not lodged between the tailgate & the bed, the tailgate latches closed on his right thumb crushing it.
Employee was rolling up welding leads when his right foot slipped and caused him to transfer his full bodyweight in a twisting motion to his left leg. This force created a simple fracture of the left fibula.
While pulling liners out of 30-134 Tunnel with an air tugger, the liner disconnected from the hook and slid down the tunnel and hit employee from behind his heels knocking him off of his feet causing him to land on his back side.
While collecting cement samples, employee slipped on ice falling to the ground. It was still snowing at the time of the accident.(Did not become reportable until he started to miss time due to surgery on 3/3/11.)

The security guard was performing a walk around patrol when a patch of snow covered ice collapsed under her foot. This created a hole 6 inches deep and 12 inches wide resulting in her falling and fracturing her right wrist.
Employee was driving a small mantrip into a crosscut and attempted to manuever past a coal hauler battery when his left front tire contacted the left rib. This caused the steering wheel to jerk and spin suddenly. The spoke of the steering wheel struck the top of his hand. A subsequent MRI indicated a fracture, and the employee was taken off work.
Employee was walking across the roadway when he stepped on a rock and rolled his right ankle. Employee did not start missing time from this injury until 12/28/2010.
While fixing liner in SAG mill, employee's right hand pinched between the liner handler and the SAG mill trunion. Xrays at ER showed right wrist fracture and employee referred to ortho. In a short cast and off work.
Employee was kneeling down to install a well head in a seal and felt pain in knee. Missing work as of 1/10/11.
EE was replacing a u-belt onto a pulley and when he was trying to slip the belt over the pulley his fingers (index & middle) got caught between the pulley and the v-belt pinching ee's fingers.
EE was walking in drift mouth when he slipped and twisted his ankle. EE was released to go back to work but chose not to.
Emplyee was operating bulk bag machine when he turned to walk away from the bagging system. He struck his right hand on the I-Beam that supports the bagger and his right knuckle was cut.
Member had left the section walking to the supply car to get some dust hose for the roof bolter, when a rock fell and hit him on the back. Member worked every shift until 2-3-2011 when he was taken off work due to increased pain in his back and shoulder.
Electrician was replacing a wheel unit on the Low-Trac. He lifted the wheel unit to install in and felt pain and a protrusion in his lower abdomen. He was diagnosed with a hernia and surgery is scheduled. He has continued to work.
Injured was standing beside roof bolter and tripped on uneven bottom, fell backwards and hit drill boom lever swinging boom over and striking right leg.
Employee stated he had moved the roof bolter cable and he said he was unaware that the operator was about to start the cable reel in order to take up the cable. When the cable moved it struck the employee caused him to lose his balance and fall. When he fell he landed on his self rescuer causing pain to the employees left side. He was told he had a contusion.
Employee was cleaning the loading area with front end loader when he ran into a low place in the pit floor that caused the machine to twist him in the seat causing pain in back.
While using his hammer to knock ice off the belt roller, his hand slipped, causing it to become caught between the belt and the skirt board rubber on the belt.
Stretching out data line in Belt/Track entry stepped in hole and overextended knee. Did not start missing until 2/28/11
EE was standing behind BT.22 mobile which was off track in a track spurr. The mobile was struck with a 20 ton motor which caused the mobile to kick sideways pinning the ee feet beneath the mobile and the rib line.
While performing decking work, employee stepped on a piece of unfitted unwelded decking, falling 17 feet to the deck below. He had a facial cut; no other visible injuries. He was asked recall questions, said he was ok and refused care. He worked rest of the day; gait, balance and other activity were normal. Early 12/22 at local ER was diagnosed with lung/rib injuries.
Moving 55 gal barrel. Injury occurred 12/21/10. Rechecked on 3/16/11 - off work since 3/16/11.
EE was pulling a cable with the cathead over his shoulder. He slipped and fell forward striking his chest on the cathead. He was taken to ER and was diagnosed with a fractured sternum.
EE stated he was holding a metal tie in position on the 60lbs rail while fellow ee was hammering it to install the lugs onto the rail. The hammering resulted in the rail being knocked from a lifting jack being used to raise the rail. The injured ee left hand was caught between the metal tie and the mine floor, causing a laceration and a fracture to his left hand
The injured bent over to pick up a scoop bucket cylinder and felt a sharp pain in his lower back. (The injured noted this to the section supervisor but did not go to the doctor). The injured continued to work & on 1/31/11 he informed his supervisor that he had to be off work & was under a doctor's care from the initial injury of 12/21/10.
Employee was standing on the pontoons of the #2 longwall face support when a rock fell from in between the #2 and #3 longwall face supports at the tip. The rock struck the top of the headgate gearbox cover and when doing so it bounced off the cover and then struck the employee on the left foot causing injury.
The employee stated that he was watching his cable on the continuous miner he was tramming. The employee stated he picked up the miner cable and started walking backwards pulling the cable to get a knot out. The employee stated that while doing this, he stepped on a rock approximately 10"" in diameter and twisted his right knee.
Moving structural beams in to place. Used a plate cart to move a beam assembly. Instead of lifting the beam from the cart with the available chain hoist, the employee dumped the beam from the cart. The beam caused the cart to roll, allowing the beam to move and fall on to the employee's left foot - striking his metatarsal guard.
EE was walking his normal work area when he tripped and started to fall. EE felt a pain in his left shoulder when he reached out to catch himself with his left arm. EE taken off work by his treating physician. First day missed: 12/28/2010
Employee was removing the planetary gear from the long wall shearer using a 3/4"" drive ratchet. The ratchet slipped off the bolt causing him to strike his right thumb on the plan line resulting in a fracture.
Employee was leaving his truck to enter the mine office/change room, he slipped on the ice and snow and fell on his left hand. The weather was snow and ice in the early AM this morning.
Employee working on a piece of equipment was pulling with wrenches, when he felt a pull and pop in his left shoulder. Lost time injury 01/25/2011 due to surgery. **Employee's surgery was cancelled to be rescheduled later. Employee remains on full duty status.
Employee was fueling up his haul truck when the compressor on the fuel truck ran out of gas. Employee began adding gasoline to the compressor and spilled some onto the hot muffler. This ignited the gasoline which caused the employee to get burned on his face and neck. Employee was taken to emergency room. Employee has 2nd degree burns on 2% of his body.
Mechanic was grinding on steel when personal grinder exploded, came apart and cut upper right thigh.
Employee was using a torch to cut a piece of pipe. While handling the pipe, it slipped out of his hand and he moved suddenly to try to catch it and it hit his hand.
Turning off compressor and bleeding line. He grabbed wrong valve and a burst of air hit him in the chest, knocked him down.
Employee states that as he was attempting to dump a load of coal from trailer, the coal stuck in trailer causing the front wheels of tractor to raise off the ground. When the coal dislodged, the trailer came down suddenly, jarring employee.
Employee was pre shifting vehicle after safety meeting, and cleaning windows on Volvo A30 haul truck, started to wash the windows, standing next to the fueling spout on the flat are provided for standing, slipped on fuel residue and fell approx 6 feet to the ground on his lower back and butt. Employee also had a headlamp on, the sun was just starting to come up.
The employee was advancing the RBM using the inch controls when a rib corner fell out striking him in the middle back area. The event resulted in a lost time injury.

Employee was roof bolting when the sleeve of his coveralls got caught on drill steel thus wrapping right arm around drill steel.
Employee was roof bolting. He stated that he twisted around to retrieve a roof bolt when he felt a pain in his back. He worked the rest of his shift and went to the emergency room on 12-21-2010 at 10:00 a.m. His was told that he had a pulled muscle.
Employee was pushing a rock with the corner of his blade of his Cat. D10 Dozer when the blade slipped off the rock, causing a quick jerk to employee's back. He was transported to the Hazard ARH where the preliminary diagnosis was a muscle strain.
A piece of loose roof fell and as he was attempting to move out of the way, he twisted his right knee, causing a strain of the right knee.

EE is a mechanic. His service truck had broken down. He was under the truck removing the drive line when dirt fell into his eye. First aid was administered. He went to the ER and eye doctor. No damage to eye. Returned to work after one day off.
employee was operating a dozer trying to remove a large rock from the pit. while trying to perform this task, he backed the dozer over a large rock and the dozer hit the ground on the other side of the rock. Employee complained of back pain.
EE was drilling a left hole with pauline bolter, when drill steel got hung up, EE grabbed steels with left hand and checked bolter suction with right hand. The steel that was hung up in the hole fell, striking the steels he was holding, causing the drill steel to strike his right hand. Resulted in a fracture to the right ring finger.
Employee tripped on a rock while walking the beltline and hurt his left knee. EMPLOYEE DID NOT START LOSING TIME UNTIL 1/8/2011 DUE TO INCREASED PAIN AND PENDING SURGERY.
Installing torque arms on miner felt pain in stomach. Employee filled out accident form but refused medical attention, and continued work until 11/10/11. Employee had surgery on 1/11/11.
Vega employee was driving a 10 Wheel dump truck following a TIC employee up a hill at Newmont. It was slippery and the truck lost traction and slid down on its side. Vega employee was taken by Newmont ambulance to Northern Nevada Regional Hospital.
Victim was pulling on cable, and fell. Did not report it until 12-17-10, when his back began to hurt. Went to dr. on 12-20-10, L-5 vertebra out of line. Has appt. with physical therapy on 01-03-2011.
Employee states that while operating the center car, he hit a hole in the road and it jarred him. He felt a pain in his lower back.
Employee was installing a draw head on his RRD motor before coupling to the rock duster. A second supply motor was coming up the track and the operator did not notice that the switch was thrown toward the side track where the employee was working. The second motor hit the RRD motor and knocked it into employee.
Injured employee was loading a 30"" X 10' vent tube into the back of a Jeffrey Ramcar. After lifting one end of tube onto tailgate, injured employee lifted other end and pushed tube. During the time he was pushing tube, he wrenched his back.
EE was struck by a piece of roof rock that fell on roof bolter and slid down his knee. Injury resulted in miner treated and required to ambulate on crutches. This report is being generated under protest to terminate citation #9006089.
Employee was walking across the section at a hard pace, when he felt a sharp pain in his lower back.
Two co-workers jumped on employees back pushing him to the ground, (horseplay) resulting in 3 broken ribs for the employee.
The employee stated that a rock fell between roof bolts, striking him on the head. Please note that the employee did not miss his first day of work until January 4, 2011.
Operator was hauling dirt from cut area. He had just dumped a load on spoil and was driving on return ramp when he drove up on berm, proceeded approximately 80 feet and then drove off of highwall, resulting in injuries and damage to equipment.
While working with a air powered hand drill, the employee dropped the drill and in an attempt to catch it a piece of metal entered his left hand palm, causing a pencil sized laceration that required medical attention to release pressure from infection that developed.
EE slipped in parking lot, fracturing clavicle and rib on left side.
EE was cleaning off side mirrors when he slipped and grabbed the handrail of the truck causing pain in his left shoulder and arm.
Employee said he was sick, had M.E.T. check him out, his B/P was 190/110. He was transported to ER. After arriving at ER employee said that while pulling cap off of Buffalo seam he caught a hard spot that jarred his neck and back. He worked for a time and the pain and stiffness continued until he got sick and vomit. X-ray was negative and Dr. took him off work starting 12/17/2010.
The accident was caused from working on a slick surface, the surface was slick from the weather conditions that we were having on the site. The worker sprained his left knee when he slipped and had his left leg caught in-between an inverted rail and rebar.
Employee was lifting a 5 gallon bucket with some concrete in it and felt something pull in his lower left side.
Employee was tightening a bolt on excavator bucket with 3/4"" impact wrench. His finger got caught between bolt and h-link causing a gash on left middle finger. Employee was taken to the nearest hospital where his finger was stitched.

The EE was walking from the feeder to the shuttle car and stepped into a hole twisting his left knee.
Injured was installing roof bolts when his drill steel became lodged and bent as he freed the steel and lowered it from the roof he reached in to grab the still and inadvertently bumped the bolters control lever rotating the steel sticking him on the inside of the right arm and hand causing a fracture to his thumb.
EE was removing a set of belt scales off the belt line #3 belt bk-26 he struck the scales with a 4lb hammer and they fell down hitting his ring finger on left hand causing an avulsion which required surgery and placed off work until the wound heals. He was wearing gloves at time of incident.
Employee went to unplug an extension cord that was plugged into a drill. He had one hand on a metal beam and touched the prong on the extension cord at the same time causing him electrical shock.
Employee was taking down MRS cable to take outby when a piece of rock approx. 4"" thick, 4"" long, and 3"" wide fell from top striking him in the back of his head, neck, shoulders and back, driving him to the ground. * Did not report prior because employee did not begin missing work until 01/06/2011.
EE was raising the head of the center bolter when a drill steel fell behind the head. The drill steel sprung back and struck the EE in the lower jaw. This resulted in a broken mandible and laceration.
Employee was handing shells to employee shooting kiln and lime from his hard hat accumulated and blew into his eye.
Employee was seen wiping the windows on the CAT-773 haul truck and about 5 to 10 minutes later he was seen by the Stockpile Operator, laying on the ground in front of the haul truck. Employee sustained a laceration to his head and broke his right wrist.
EE was loading a face when rock came off of face and struck EE in head and middle of back. EE has been hospitalized. Extent of injuries unknown at this time.
The employee got off a front-end loader and walked toward the classifier when he slipped on a patch of ice. The employee fell to the ground landing on his back and also hitting the back of his head. The employee was taken to a local hospital where he was treated and released for a possible neck & back sprain.
Emp bent over, opened a 45 pound metal cover for gas meter and felt pain in lower back upon pulling and turning to move cover. Investigation completed and determined work related on 12/21/2010.
(Report received from Coal River on 12/16/10) - Employee stated that while hanging curtain in the #1 entry on C-section, a piece of draw rock 15"" X 12"" X 1"" thick fell and struck him on the right shoulder.
EE was helping move miner cable out of roadway in the #5 entry, when they started moving the miner cable caught his feet knocking them out from under him. This caused him to fall on his left hip, hurting his lower back. The EE did not go and see a doctor until 12/17/10, the doctor took him off of work.
Employee stated that he was getting onto a dozer. He had one foot on the step of the push beam and the other foot on top of the push beam. His right foot slipped off of the step causing him to fall down and strike his right rib cage against the top of the push beam resulting in a fractured right rib.
An employee was closing doors on the train cars. The employee was pulling down on a bar to close the car door and it slipped causing the employee to fall backwards onto his right foot. The employee notified PBS Coals, Inc. on December 16, 2010 that he would need surgery to repair the broken fibula bone.
Slipped and fell on ice while walking.
The employee was removing a hydraulic line on a hydraulic cylinder, when the pressure in the system hit him in the face hurting his eye. The hydraulic fluid knocked off his safety glasses and hard hat.
Employee was operating a roof bolter when a piece of draw slate fell striking his right hand resulting in a laceration that required stitches to close.
Loading structure into scoop bucket, EE told the operator to pull ram in to tighten pull chain. EE had his index finger in back of ram. He had swelling to his finger but x-rays revealed no injury. He stayed off for a follow up with specialist and no injury was found. No therapy or treatment has been scheduled. Due to soreness, he remains off work.
EE was passing a bucket of sealant thru a mandoor in a Kennedy stopping when he pulled a lower back muscle. EE went to the clinic was checked out and released and RTW. EE missed work 2 days later, returned to the Dr. EE doing P.T.
Employee was hitting the clogged chute and pinched his ring finger between the moving sledge hammer handle and the vibrator. He felt the impact but thought it insignificant. A few minutes later he noticed some blood on his finger. No problems with the injury until 12/28/10.
EE's legs were taken out from under him by the roof bolter cable.
Employee was helping make a belt splice when he jerked on the splicing tool and felt pain in the middle of his back. He continue to work, the first day he missed work was 12/14/2010.
EE hit his shin when he slipped while climbing onto a Cat 938 loader. The result was a minor cut and bruise on his shin. This accident occurred on 12/11/2010 but did not become reportable until 12/28/2010 because of an infection. EE was admitted to the hospital to have the infection treated and fluid drained.
Employee was walking through plant and slipped on ice and snow and fell.
Employee was helping load shot. Carrying buckets of stone to use as stemming. He felt pull to groin area. Thought he was going to be Ok but could not get out of bed next morning.
Employee was working in the shop cutting metal for a dozer blade, when his pants caught on fire, causing severe burns to his legs and groin. Employee was transported to local hospital and then transferred to a hospital with a burn facility.
Miner was transferring limestone to a pallet when a piece slipped from his hand. It fell striking his left ankle area.
Employee was bolting and when he reached to get the steel out of the roof he sprained his shoulder. Employee started missing 02-04-11.
He was drilling a hole with a Cobra Drill, as he was attempting to finish the hole the steel fell out of the roof and struck him on the left hand above the little finger. He was removed for surgery to repair a fracture on 12/11/2010.
Employee was removing leg transducer on shield and had cut the pressure valve off and was reaching for the valve when it blew off and was hit in the face with high pressure soluable oil.
WAS CLEANING AROUND PLANT AND SAW THE COARSE MATERIAL WASHER HAD STOPPED. WAS GOING TO ELECTRICAL SHED TO SHUT OFF POWER AND ROLLED HIS ANKLE.
Employee was placing rock in core when he was jarred causing his neck to become stiff.
Employee was pushing the ash ring when he pulled muscles in his lower back.
Driver entered bed of truck with step ladder to prep. bed for hauling. While exiting bed slipped from ladder falling to ground approx. 4 feet.
Employee walked under an overcast and he raised up too soon and hit the last I beam. Employee started missing 04-18-11.
Employee was carrying filters through filter trailer, slipped and fell on shoulder.
Putting hold in with jackhammer and fell against jackhammer handle. Bruised ribs right side.
EE was setting bits on the Miner in the #6 entry inby spad #1334. He turned around to put an old bit in a bucket and took a catch in his back. He was transported to Summersville Regional Hospital by ambulance where the ER doctor placed him off work for a week to rest his back.
Worker said he was getting mud off of a roller with a bar and somehow got his hand caught between roller and conveyor belt. He is trained not to work on Feed Hopper while in operation.
Employee was struck in the head and neck with a piece of falling mud while changing a tire.
The employee was sitting in a rubber tire mantrip with one leg on the ground. The operator advanced the mantrip forward causing a twist in the lower back. The injury was not reported until 12/13/10.

EE began losing time for pain to his right leg on the next shift (12/09/10). An MRI was done on 12/13/10 (report attached). He will undergo surgery to correct the condition. This is being claimed as an occupational illness due to degeneration.
While changing a level wine on #15 car, EE and another EE were helping lift level wine. He strained his lower back.

The employee was originally diagnosed as a wrist sprain. The employee continued to work his regular job until 02/23/2011 when he had an MRI diagnosing tendon damage. Employee had surgery on 02/24/2011 and is now off work.
Using Scoop and Chain and Pulling TailPiece to pull the belt back. The chain broke and the tailpiece lunged and hit EE on the lower portion of his right leg. Employee was removed from the mine and taken to the Hospital. Swelling, Bruising of lower portion of leg. Not yet known if any bones were fractured/broken.
Employee alleged strain injury to right shoulder when performing pre-trip inspection on rock truck.
Loader operator was going to get a bucket of pit run to feed into the plant. As he lowered his bucket to the ground while proceeding forward he hit something solid with the front of the loader bucket stopping him abruptly.
While the foreman was walking accross the section he was hit by a piece of draw rock measuring 52"" by 24"" and 0-2"" in thickness. He suffered contusions to his back and right leg.

EE is claiming an injury that supposedly occurred back in October or December that involves a strain to his left shoulder. A sprain occurred while opening flapper gate on rip rap flapper lever hopper. It is being investigated. We were first notified on 2-14-11 when injury supposedly happened on 12-6-10. The operator was cleaning the area around the short head cone crusher at the rock plant when a piece of channel iron weighing about 20 lbs slid down the handrail along the catwalk and landed on his left foot. Two days later (Dec.8), he reported swelling around the ankle and was taken to an urgent care facility for medical treatment.
Employee was cutting a piece of 4 inch drill steel when the piece fell off the support and struck the employee's left foot, fracturing the second metatarsal bone.

Employee states he was assisting in installing a concussion door. As he was holding the door in place, it was hit with a blast of air, knocking him backwards and causing him to fall. He received a laceration on his head requiring six stitches and a dislocated finger on his right hand. Employee received medical care and was released for work.
Surface Equipment Operator prepared to tie tractor trailer lid down. After he threw the rope over the top, he felt pain in his chest. This trailer does not have a permanent mechanical lid locking system like the other trailers. Because of this, a rope must be thrown over the top of the trailer so the lid can be tied from both sides. Employee suffered a pectoral minor tear
After performing maintenance work on the bucket in the HP building top floor, ee was descending a 12'fixed vertical ladder. During the descent ee's hand slipped resulting in his falling from one level to another to the grated fiberglass floor below (approximately 11').
Employee was spot bolting and left his hand on tray of bolter. A piece of rock fell on his left hand.

EE was stepping thru a man door at 5 West #2 block belt stopping. EE stepped on a piece of steel cable causing him to slip. Felt pain in left knee and right hip.
Employee was going down ladder to ore chute in skip pocket, foot slipped on rung, employee caught himself on the next rung and felt his knee to pop. Employee required surgery on 04/07/11 and began losing time.

Coal rib bump occured the miner operator had graded to the face and bumped into the coal face. The operator was standing outby the miner tail when he was hit by coal from behind the screened rib. The operator was taken to the hospital. A metal plate and screws were used to repair his broken left shoulder. He was released from the hospital 2 days later.
EMPLOYEE WAS STANDING ON THE NORTH SIDE OF TAIL PULLEY RESTING FOOT ON THE TOE PLATE, UNDER THE TAIL PULLEY SUPPORT FRAME. BELT TRAVELING UPWARDS CAUGHT THE TOE OF THE BOOT AND PULLED HIS FOOT INTO THE STRUCTURAL IRON.

Exiting from a 740 end dump to meet with supervisor, victim informed supervisor he had fallen from bottom step dump. Victim was okay and then assigned to operate dozer. After appx. 15 min of dozer operation victim informed supv. of numbness of hands. Sup again met with victim and while transporting to office victim started with convulsions. An ambulance was summoned.
While installing roof bolts on the 15 Headgate Section #1 entry face, the EE was struck by a roof bolt plate on the palm of his right hand causing a laceration which required 8 sutures to close the wound.
Employee was carrying an Idler Bearing for a shuttlecar out to set it down on the mantrip. As he set it down, he felt a sharp pain in his lower back.

Employee was exiting pickup truck to apply wheel chock, which was secured to the bed of the truck with a piece of 1/4"" cotton blend rope. Upon completing placement of wheel chock, employee turned to walk away and caught right foot on rope, causing him to fall forward; resulting in fractures to right and left wrists.
Employee stated he was scaling and sat the scaling bar on top of his foot. When they moved the cage the bar contacted the roof and pushed the bar onto his toe. His right middle toe was cut near the joint. He is currently missing time from work.
EE was watching a track rail being bent with a rail bender the bender was being pressurized when the rail broke striking ee on the right shin. EE suffer a bruised shin bone.
Employee was shoveling in the Sand Plant Tunnel when he tried to remove a piece of frozen material from a chute. As he leaned over and tried to pull the material free he felt a sting in his lower stomach. Medical diagnosis is a Direct Inguinal Hernia. Employee was able to return to work with restrictions.
EE experienced pain in his left knee while walking . On 02/02/2011 EE had surgery on his left knee. It has been accepted as a work related injury.

Employee attempted to start up lime ball mill when he noticed that the classifier screen was plugged with lime. He used the water hose in an attempt to clean the screen. The water mixed with lime splashed onto him and into his eyes (employee was wearing safety glasses w/side shields).
EE was helping another ee repair the suction on the drill head of a roof bolter where the repair were completed ee inadvertently lowered the drill head on ee leg causing swelling or bruising to the upper right leg & ankle.
He was changing oil in the WB unit at the wash plant. He felt a pull in the middle of his back when he lifted a 5-gallon drum of oil.
During roof bolting operations, the individual states he had his hand on canopy handle and when he swung the drill head out, the canopy came in contact with the roof mashing his left ring and pinky fingers.
Employee was pulling a chain and it struck her on the left wrist and fingers. She was evaluated by a Physician and a splint/sling was applied.
EE was working to clear a pluggage on a bauxite bin when his left middle finger was caught between a water lance and the bin structure. EE was taken to local hospital where Xray was negative for a fracture. On January 17, 2011, Orthopedic Surgeon diagnosed fracture of the left middle finger, making this a Recordable Injury charged to 2010.
Employee was making his inspection of his coal truck and when he went around the truck he slipped on the ice and fell to the ground. He injured his left shoulder. Contusion/impingement.

On 12/13/2010, the employee was operating a continuous miner, and he stated that he went to lift a bucket of miner bits and felt a pain in his left shoulder. He later tried to pry a rock off the continuous miner and the pain got worse. He continued working, a MRI was done on 2/22/11 which showed a torn labrum. The IE had surgery on 2/28/2011.
Contract driver was manually cranking his tarp open, when the crank handle began to ""free roll"" striking the contract driver in right lower arm resulting in a contusion.
Employee was walking on inclined catwalk, when he felt something pop in his upper leg. Physician diagnosed as torn hamstring. There was no slip or trip involved.
EE was walking #4 Belt South Mains when he heard a noise behind him. EE looked over his shoulder when he turned back around he walked into a fire valve, striking his upper lip requiring 12 sutures.
Employee was taking the slack out of the AFC chain when the chain caught on the head drive. Employee tried to pull the chain free when he felt a tear in his groin
Employee was working at Transloading sealing up top hatches on railroad cars. He was wearing a full body harness attached to retractable lanyard along with his PPE. While walking on the catwalk of the car, his lanyard caught on the handrail, making him lose his balance and fall from the walkway. He fell against the car resulting in the injuries-3 Fractured ribs.
Miner was walking down steps, at last step he twisted knee, tearing ligaments.
Initial accident was on 12/8/10 when employee helped lift a barrel of chains. approx 200 lbs. Employee felt a strain. He refused treatment at the time. Next morning manager sent him for a check up - possible hernia. 3 weeks for workman's comp to agree for follow-up with doc. Employee went to doc on 1/4/2011. Dr. suggested surgery on the 17th. no loss time till 1/17/2011.
Employee was struck in the calf by the pin of a shackle resulting in a bruise to the leg. Originally diagnosed as a non occupationial due to personal medical condition on 12/15/10 it was determined to be occupational.
Employee was exiting from the back side of the headerline, slipped and fell on knee.

Employee was walking from Midway building to the ready line, where his truck was parked. In between the South side of the midway and the ready line the employee slipped on a patch of snow covered ice causing him to fall on his left shoulder. Employee was placed on restricted duty.
An employee and was reaching with his right hand to clear a rock that was located in the bretby tray. It appears that a rock was propelled from the head gate drum striking the employee in the left hand and chest. The employee was diagnosed with having sustained a severe strain to the wrist and lacerations to the thumb. On January 3rd, he was placed on restricted duty for 10 days.
The employee swept up a small pile of trash. He left the area and took a break. When he returned to the area, he stepped on part of the trash pile. His right foot slipped on a plastic jug cap (in the pile). Due to the twisting motion, it caused an ankle sprain.
Employee slipped and fell on icy concrete in the steam bay. As a result there was green stick fracture to right fibula.

Replacing 3-ways on dump chains and the bridle pins (1 3/4"" x 9"") He picked the bridle pin (with the top ""keeper"" in place) up with his right hand, by the top(above the ""keeper"") He started to drop it into the hole, his left hand was next to the hole, the bridle pin slipped out of his hand and left thumb was caught between the pin keeper and the clevis.
While placing pipe on a scissor lift, pipe fell about 6 inches lacerating the employee's left pinky finger. Four stitches required to close wound.

While exiting a door to the outside employee slipped on a piece of siding which was covered in snow causing some discomfort in his left knee. The employee was not initially in any pain and did not seek medical attention but after several days the knee began to bother him. Employee sought medical attention on 01/22/2011 and then received restricted work activity.
The miner was assisting in the installation of a Heintzmann beam when it fell from the king rail and struck him on the left hand breaking one of his fingers.

Employee was operating a boom truck when the auto kick out on cable got in a bind. When he attempted to free the auto kick out it slipped and mashed his left ring finger.
Barring down loose material prior to bolting back in the 4300-275. Material came down and hit employee on right wrist (between arm guard and glove) causing laceration.

Employee was stepping down from a high wall miner conveyor car, stepped into a hole or depression on the pit bottom. His foot turned under and inward as he placed his full body weight on the ankle. He felt immediate pain in the ankle area. He was transported to the emergency room and diagnosed with a bone chip in the ankle. Employee is a lost time.
replacing return rollers on the pebble stacker conveyor. He was on the south side about a 1/4 of the way up. Employee was lying on right side of grading, facing the conveyor with his head towards the top of incline. He was swinging a 4 pound hammer to knock the rollers out. He felt a sharp pain in his right shoulder. He notified his supervisor and finished the day up.
Scraper operator had filled bowl with basalt and started to pull away, as he was moving forward, the scraper slid on snow. Scraper operator slowed down and stopped abruptly as dozer operator approached from behind to push scraper, hitting stinger (push pad) on scraper causing injury to scraper operator.
A precipitation operator was putting a caustic hose on the bottom of tank #70 while standing on a scissor lift. While tapping the hose connection with a 4 lb. hammer, he struck his middle finger that was holding the bleeder.
The continuous miner was hung up on a rock ledge. They cribbed up the ripper head so they could bow it down and raise the frame of the miner to get some wood under the cats of the miner, while the injured was pushing wood under the cats of the miner the cribs under the ripper heads crushed/rolled out and caught the EE's right thumb between the cat and the crib.
Employee tripped on ladder base and fell landing on right knee cap. Initially employee received medical only treatment but on 1/20/11 the employee received medical treatment and restrictions for a short period of time.
EE was walking through the center of a walk through roof bolter when he hit his back on a bent down roof strap. This caused a laceration to his right shoulder. (6 stitches)
The employee was pull-starting a generator in the back of a pickup-truck. The handle came un-tied, and the employee fell backward out of the truck bed. She suffered a sprained neck and was placed on light duty by a doctor. No permanent damage is suspected.

EE was operating a fork truck and crossing a set of railroad tracks when the forks or load caught a rail causing the fork truck to stop abruptly. The employee was caught by his seatbelt but he bent forward and hit his chest on the steering wheel.
Injured was striking 1A tank with a hammer to attempt to get material to flow. When injured struck tank, lime dust fell onto him from above, entering into his right eye past his goggles.
Employee was negotiating a corner in a John Deere Gator on a slope underground and the right tires climbed the berm guarding the right rib after turning too short. The Gator turned over on it's side and the employee's left leg was injured in the accident.
Employee was putting firewood into woodburning stove; he jammed left middle finger, causing a fracture in the fingertip.

Employee was using a torch to cut a guard that was on the ground. He turned around and slipped, and reached out with his right hand to catch himself, and injured his right wrist.
The employee was installing a 5' torque-tension roof bolt in a 40"" mining height which required the bolt to be bent. The operator had started the bolt into the mine roof and was attempting to straighten the bolt when he felt pain in his back.
Employee was trying to close and latch dozer door frozen due to cold weather when he slammed the door on his finger.
Employee was cutting steel on band saw and dropped a piece that was on the roller bed onto his foot. Physician diagnosed as contusion, prescribed short term limited work restriction.
H.R. Manager was walking away from the office to get her car and slipped and fell on the paved visitor parking area west of the office. When her feet went out beneath her she reached back with her left arm to catch her fall and had an injury to her left wrist. The injury required med. intervention that day and restricted duty thereafter during recovery period.
Walking and tripped on strap & debris fell on left knee bumping it & head(hard hat on) on pallet of empty bulk bags. Inflammation in neck & soreness in left shoulder and knee. Able to work with restrictions.
Employee strained his back while lifting five drill rods back up from the ground and place against the rib. They weigh approxiamately 60 pounds each.
Employee was walking over a snow bank when he sunk into the snow causing him to strain his left leg. He was evaluated by a physician and placed on restrictive duty starting 12/15/10.
Mechanic helper tossed a wire rope sling to coworker who was standing by the pipe they were preparing to lift. Inadvertently, the sling got caught on the boom truck rear bumper abruptly stopping his motion causing injury to his left shoulder and arm.
Not sure as to exact cause of accident, however, during the day, the EE noticed that his Left Knee was getting stiff. After being examined by a doctor, he was sent for an MRI on 12/17/2010. He was diagnosed with grade 4 tear of the lateral meniscus of his left knee.
Employee was climbing ladder on #20 drill when she felt a pop in her right foot with pain,resulting in right ankle sprain.
Injured was attempting to rotate the #1 turnhead to the coal silo position. As he started rotating the turnhead, it unexpectedly moved quickly causing injured to feel a ""pull"" in his back. Injured did not request medical treatment initially. Injured was given work restrictions on 2/14/2011 when the condition did not improve.
Employee stated he was walking and stepped off a small ledge on the floor and twisted his lower back.
After working to thaw out a come-a-long latch mechanism, EE proceeded to move the ratchet handle in order to crank up loading chute, when the ratchet handle slipped, striking EE on left eye socket area causing a contusion.
Two workers were setting up the crane to make lifts on a crusher. The operators cab was raised to let snow & ice slide off the top glass. The cab stuck in the up position. EE kneeled down & reached under the cab & loosened a hydraulic hose to lower the cab. The cab gently lowered onto his back and held him for a few minutes until co-worker was able to lift the cab to let him out.
EE WAS HELPING SET TIMBERS ON THE OFFSIDE OF THE BELTLINE. THEY WERE SLIDING THE TIMBERS ACROSS THE BELT ON THE BELT RAILS. EE GOT HIS FINGERS MASHED BETWEEN THE END OF A TIMER AND THE COAL RIB.
While removing the cribbing from under a haul truck the mechanic was stacking the wood on the side when he smashed his right hand ring and middle finger between two pieces of cribbing.
A brow fell and hit employee while he was operating a miner resulting in pain in the center of his back.
Employee was using a jackhammer to clean scale from a kiln, got dust in his eyes resulting in burning.
Was loading Rail Cars and the loading doors were frozen to the top of the car. Wrenched his lower back trying to open these doors.
Slipped and tripped on soft, muddy ground, straining knee. Medical treatment obtained on Monday, 12/13/10, at which time work restrictions were given by doctor.
Employee states that while bolting top in the #4 entry, 002 section a piece of draw rock fell and struck him on the bridge of the nose. The draw rock caused a cut to the bridge of his nose and chipped a tooth.
Employee was talking to another employee when other employee's cell phone rang. When employee stepped backwards to give other employee privacy, he tripped & fell over a landscape rock striking right shoulder on the ground. Employee was on a previously scheduled vacation from 12/11/10 to 1/9/11 and returned to work on restricted duty on 1/10/11.
Injured employee and a helper were loading scrap into a scrap bin. They both picked up a scrap idler and were in the process of throwing it into the bin, when the helper let go of his end. The motion of the piece of scrap being let go caught the injured employee's left thumb straining it.
Employee was replacing a wear plate inside MI-0503 when the plate slipped he instinctively tried to catch it. The plate weighing 70# smashed his finger (3rd digit on left hand)between the plate and the ground. He was immediately sent for medical evaluation.
Employee was picking up a jackleg drill after finishing drilling a knee hole when he felt a very sharp pain in his lower back.
Employee was using two wrenches to break loose a pipe fitting. As he placed pressure/weight on the wrenches he heard and felt a ""pop"" in his right shoulder. Strain/sprain to right shoulder.
Employee stepped off of stairway onto a concrete slab and twisted their ankle. This resulted in a hairline fracture to the bone.
EE states he stepped off a skid steer onto uneven ground, twisting his right ankle and causing muscle strain. EE was placed on modified duty 12/9/10 and released to full duty on 12/16/10. There is no lost time associated with this injury.
EE was working on core drill and was pulling pipe from the drill hole. When he pulled on the pipe he strained his left wrist.
While Employee was walking down the stairs of the 19 dragline he missed a step causing his right leg to miss the rest of the steps; hitting the dragline shoe. When the right leg slipped, employees left foot got caught in-between two steps. Employee was stuck for a couple of minutes and then was able to get himself up.
Emp. was adjusting gate in storage tunnel when the clean up crew was moving the suction hose from the vacuum truck and knocked his feet from under him and he fell on his knee and shoulder. Emp. stated that it was very dusty in the tunnel and could hear the hose moving but didn't know it was that close to his feet. Taken to the doctor on 12/10/10 and placed on restrictions.
EE climbed down from his haul truck and stepped on a piece of rock, causing him to fall backwards.When he reached back to break his fall he caused a slight fracture to his right wrist (distal radius).
Employee was picking up a roll of skirt rubber when he heard a pop in his right shoulder. He went to the doctor and was diagnosed with a shoulder strain. He returned to work that same day on light duty.
Injured was lifting cement blocks and tripped over another block twisting right ankle. Causing restricted duty.
Employee lifted a bucket full of dried cement, in doing so he experienced discomfort in his right shoulder resulting in a sprain.
A custom made guard was made for a pit. The emp. kicked the 15lb guard to knock in place. The heal of his boot caught the guard. The emp. felt some pain to his right knee. Emp. continued working however later in the day his right knee swelled with increased pain and he was unable to get out the man trip. Emp. taken to doctor and placed on restrictions.
Employee was holding onto a 2"" air hose, and his co-worker opened the air valve to clean out the air line. The resulting pressure was more than employee could handle and the hose struck him in the left knee.
Allegedly employee stepped down off of sampler and reinjured his knee. Contractor was notified of this incident on 1/5/11.
Enlarging the opening failed then ee used a pry bar. While poking the obstruction the bar glanced off the obstruction contacting the moving conveyor belt kicking the bar and his hand that was holding the end of the bar up against a horizontal support between the skirt boards. This resulted in the end of the bar piercing the palm of his right hand.
As employee was driving his B30 Euclid haul unit, he hit a bump in the road, causing soreness/stiffness in his neck and right shoulder.
Employee was punching out a brass bushing on a drill machine when he missed the punch and smashed his left index finger between the hammer and the punch causing a blood blister and swelling to the finger.
Cutting stone with a large hammer, rock split and fell towards employee. Employee strained body/back trying to get out of the way.
Employee was stepping through an air lock door when the outby airlock wall partially collapsed causing a pressure increase that caught the ee left leg between the man door and the man door frame resulting in a contusion.
Dumping small lab hopper and associate hit the frozen sand with hand to free material. The frozen sand came out and the hopper weight shifted and hopper lid stuck right hand index finger, peeling skin back. Taken to urgent care and received 5 stitches.
Employee was assembling belt structure for a belt move and felt a pain in his right shoulder. He missed his first day of work over the accident on 12/20/2010.
Plant Operator strained his low back on Monday, 12/06/10, while shoveling sand. On Friday, 12/10/10, the Plant Operator was carrying a 20 lb Come-Along up some stairs causing further strain to his low back. On Monday, 12/13/10, the employee reported the injury to his supervisor and that he was suffering from severe pain in his low back.
Worker was lifting a piece of stone 2'X 4"" when he lost his footing and slipped. He twisted to regain his balance and injured his R wrist.
When employee stepped down off chippy hoist at 4450 station the ground was uneven, the skip was about 8"" higher that the station floor. He felt a pain in his left foot and burning sensation.

Employee tripped on deck floor grating and while falling, reached out with left hand to arrest fall. The trauma from grabbing railing pulled something in upper left arm. (Employee is left handed)
Employee was installing a cable bolt when he inadvertently struck the joy stick, causing the bolt to spin and struck employee on his right wrist and forearm. Restricted duty injury.
Employee's hand slipped while putting inner tube back into the hole. This caused the employee to pinch his fingers between the rod and the latch head. The weight of the tube caused a laceration of the left ring finger and the left pinky finger of the employee.
Employee slipped on second step coming down a flight of stairs, tried to catch self on the way down, straining right shoulder, right elbow, and bruising right knee. Employee states there was no snow/ice build-up on stairs.
Performing maintenance on bulk loading spout. Adjusted cable & when spout began traveling up cable, the attachment came apart & eye bolt scraped right middle finger, causing laceration. Required 4 sutures and restricted with no use of right hand.
Employee was exiting his vehicle and slipped on ice. Employee was taken to the clinic due to shoulder pain and was placed on modified duty.
While employee was operating the continuous miner in the #3 face a piece of rock was thrown back off the cutter head and struck the ee in the RT eye and nose. It was reported that the ee had his safety glasses on and they were knocked off and/or broken.
EE was changing bit on drill and when he was putting chain wrench away felt pain in right hand.
