Roof fall found on 1-31-2011. Not above anchorage, not impeding ventilation. Does not impede travel.
IGNITION: The Shearer was cutting at #10 Shield when a LW Helper saw and orange flame from around the HG drum. The flame lasted about 10 seconds and went out on its own. The shearer operator did not see the flame, only the LW Helper which only had 3 months experience.
Elevator stalled between the 1st and 2nd floors for 2 hours 50 minutes.
An unplanned roof fall occurred on Cross-Cut 36 Entry 6 Main West 1 Secondary Escape way. The roof fall measured approximately 60' in length, 18' in width, and 8' of thickness.
Unplanned roof fall occurred inby feeder on MMU 004-0, from 7 right crosscut to the #9 entry. Fall was approximately 95 ft length, 19 ft wide and 8 ft in height. No injuries, entrapment or equipment involved. Last open crosscut ventilation affected and travel way. Developed 1/31/2011.
Unintentional roof fall on #3 unit's roadway, xc 12 #6 entry.
A roof fall was discovered in the 3rd Northwest main xc-96, spad # 78+10 in the intersection of #7 entry and between #7 and #8 entries. The size of the fall was approximately length- 30ft,width- 19ft and height- 6ft plus.
Employees were cutting in our 3c crusher when a piece of slag dropped down and started some rubber on fire. The fire was put out with water hoses and no injuries occurred as a result of the incident. ***This is the 2nd time that I have reported this. The 1st time was online on Feb 2nd but it was apparently lost in the system.
An unintentional roof occurred in all three approaches to the Cut-Thru EP. The fall is located inby spad 12370 in the no 1 entry, spad 12371 in the no 2 entry, and inby 12368 in the no 3 entry. No injuries occurred, escape ways & ventilation were not affected by the fall. The fall measured 20 feet wide X 7 feet high. The operator was unable to determine the length of the fall.
At 9:28pm the control room operator received an alarm that the East rupture vent door for the #1 coal mill system was open. The primary air fan belts had slipped causing the primary air to slow and then resume normal speed. This caused a low sweeping velocity in the piping and was likely the ignition source.
A roof fall occurred on the 1st. 54 S/R at 24+00. The area had been supported with 6' tension bolts, 8' cable bolts and truss bolts. The apparent cause was an undetected slip in the mine roof. An action plan for rehab. of this area has been submitted and approved by the district office on 1/31/2011.
A roof fall occurred in the #11 seam in the 2 Main North at crosscut #115 in the intersection of #6 entry. The fall measured approximately 25'x18'x6'. The area was supported with 4' roof bolts.
IGNITION: Methane ignition on #2 LW at Shield 87 involving TG drum. Operator cutting coal headed to HG with 80% methane on monitor. Flame was orange and about 12 "" high and lasted approximately 2 to 5 minutes, it was extinguished with the water sprays on the TG drum. No injuries were incurred.
An unplanned roof fall occurred along the #10 belt in the #3 entry at break #6, 1,700' from the active face at spad #9192.
A roof fall occurred in the 2nd North Sub-Main in #4 entry inby the intersection of spad #5+60 to outby spad #7+00 due to a hidden slip in the area. This is an area of old workings, no one was in the area and no ventilation was affected. The fall measured app. 120' long by 18'-20' wide by 8' high.
On January 27, 2011; RTD354 (rubber tire dozer) was cleaning the 1990 floor while the shovel in the area was temporarily down. While making a push along the highwall at 12:17 AM, the operator heard a loud noise which he assumed was a front tire failure. When he investigated the noise, he found what appeared to be a partial booster on the ground.
Elevator stalled between 1st and 2nd floors for 45 minutes.
Unplanned roof fall was discovered in the Old #1 Panel in #4 entry. The fall was outby spad # 1073 for 3 crosscuts. Fall was approx. 19 ft in width, 300 ft length and 10 ft in height. Area was developed in 7/2009. No injuries, entrapment or equipment involved. No escape way, ventilation or travel way affected.
A roof fall was discovered in the 3K panel #7 entry, xc-34, spad #22+95. The approximate size of the fall was length-19ft, width-19ft and height 6 1/2 ft.
Unplanned roof fall was discovered in the Old # 1 Panel in #5 entry. The fall was outby spad #1054 in intersection. Fall was approximately 19 ft in width, 19 ft in length & 10-12 ft in height. Area was developed in 7/2009. No injuries, entrapment or equipment involved. No escapeway, ventilation or travelway affected.

An unplanned roof fall was discovered by the mine examiner on his weekly air way examination. Fall is a contuniation of a fall at this site at spad #1461 in #6 entry return. This fall will not be rehabilitated. Fall measured 18' L X 18' W X 6' H.
At approximately 12:30 am several large pieces of the high wall dislodged due to the recent freezing and thawing. The rock struck the top of the belt entry canopy,crushing it and damaging the belt structure under it. A plan to remove the damaged canopy and belt structure was put in place and approved by MSHA and the OMSL. There were no injuries in conjunction with this incident.
A roof fall was discovered on this date. The fall is between the #22 & #23 crosscuts in the #4 entry on 2-East Return. The fall measured 70' in length, 20' in width and was 5' in thickness. No persons or equipment was affected. The area was originally supported with 5' HY-Tech bolts. Thea area was dangered off.
A mixture of coal and petroleum coke started to spontaneous combust in the #2 Coal bin. This is being reported because the fire lasted more than 30 minutes.
An unplanned roof fall occurred at -2 block in the crosscut between #1 and #2 entry. The fall measured approximately 35' long and was above the anchorage horizon of the bolts.
The automatic doors on the Somerset Portal elevator stopped working when the motor controlling the door opener circuit failed. The motor was repaired and replaced by the elevator repairmen. The elevator was returned to service on 1/27/2011 at 2:45PM. There were no persons injured as a result of this occurrence.
A roof fall was discovered by shift foreman on old 3rd left #6 Block in #2 Entry. Area was timbered and dangered off. No Equipment was involved and no interuption in ventilation.
A roof fall occured on the 1st right section in the #3 heading, 1 break inby spad #6029. The fall was approx. 8' X 18' X 22. No persons injured and no equipment was involved.
While using a burning bar to remove the first liner in the last row of the concave, slag fell through to the surge pocket. Lube lines below the concave and pinion were covered with belting for protection. Slag fell directly onto the protective belting and ignited the rubber. Damage was minimal due to placement of the protective belting over lube hoses.
The 8 haulage elevator was down more than thirty minutes when a piece of ice fell from the side of the shaft and damaged the pit stop switch.
Continuation of a previously reported roof fall at spad #3067 was discovered. Fall measured 25' L X 18' W X 6' H. This area will not be rehabilitated.
A roof fall was discovered on this date. The fall was located in the 3-South return at crosscut #4 in the #1 entry. The fall measured 40' by 20' and was 6' in thickness. The area was originally supported with 5' torque tension bolts. The area was dangered off. No equipment or persons was affected.
A non-injury roof fall accident was reported at 12:10 am 1-24-2010 to MSHA, The non-injury roof fall was at the old #5 beltline break 3. Length of fall is 30' width is 20' and height is 5' to 6' KY OMSL investigated the Non-injury roof fall.
Unplanned roof fall on the OO2 unit #24 belt outby spad #13145 and inby spad #13400 fall is approx 46' long and 19'5"" wide and 6' to 8' high
While starting to drive a slope. The first shot was taken and bolted. After the first shot was bolted a second round was drilled and shot. The foreman noticed the brow working, it was decided to keep employees back and watch the brow the brow fell along with the bolts that had been installed.
The slope hoist blower motor quit running thus taking the slope hoist out of service. This prevented personnel from being hoisted out of the mine. No injuries as a result of this incident.
An unplanned roof fall was found 6 breaks inside the mine @ No.31 survey station in the No. 4 belt entry. The fall was found by Ricky Carter. This fall will be cleaned up and rehabilitated.
An unplanned roof fall occurred in the Roadfork #51 mine, 1 brk inby spad # 1173 that measured 22' long x 18' wide x 6-8' high. No injuries were reported. The area was developed on 7-1-10.
During the routine service hoist examination, the examiner discovered deterioration to the no. 2 tail rope.
Contractor was cutting I Beam to remove from plant, when spark got in between building tin and began to smoke. Plant workers and contractor began putting water between tin and extinguished smoke.
A roof fall was found in the return off MMU 003-0. It was approximately 40' long by 20' wide by 10' high. The fall occurred in an intersection and blocked travel in the entry. The fall will be cleaned up.
A possible friction ignition occurred while mining in the #3 entry on the 004-0 MMU. An investigation is ongoing at this time. An oil-like substance of unknown type was encountered there and is being evaluated as a possible contributing factor. No persons, ventilation, or equipment was affected.
A loose wire on the #3 gear contractor prevented the hoist from operating. An electrician was called and the wire was tightened. Miners remained underground during the repair due to surface weather conditions. The hoist was restored to service at 4:55 am.
Cause unknown Sherriff Dept. is investigating wooden storage shed burned to the ground.
A unplanned roof fall occurred in the # 5 entry of the 001-0 section, this fall was 40' from active face. Fall measured 30' L. X 20' W. X 6' H. No eq. or injuries involved. Area was supported with 60"" resin bolts along with 10' cable bolts. Fall occurred in Neutral Entry, ventilation and passage of person were not affected. Federal and State Agencies were notified immediately.
An ignition occurred on the 9 West longwall face at #107 shield. A orange flame was observed at the mine roof that lasted about one minute around the tailgate shearer drum. The area was checked for methane which was .015% CH4. The shearer was stopped and power knocked. Cause may have been worn bits on the shearer drum.
Plant personnel noticed an increase in flow from the seepage area that was reported on 1/8/2012. Upon discovery it was immediately reported to MSHA and we instructed MSHA at the 1-800 number that this had been previously reported to MSHA but we were reporting the increase in flow.
A roof fall occurred on the #4 beltline at x-cut 30 in the 2nd west mains headings. The fall was at an intersection. It measured approx 35 feet long x 35' wide. It was approx 11 feet high at the highest point to the bottom.
Employee was in Elevator when the elevator stopped working, maintenance was called and elevator was repaired and the employee was not injured.
A roof fall occurred on the 1st NW Submains near the mouth of NE-1 panel, that is a continuation of a previous fall that occurred in 2010, that now is approxiamately 120'L X 19'W X 5+'H, the fall is located near spad #34328. A larger area was dangered off and the NE-1 EP was relocated.
An unplanned roof fall was discovered in #6 entry of the 4A Belt Mains tow breaks outby spad #1104, between break 47 and 48 (did not extend into intersections). Fall was approximately 55 ft long, 18 ft wide and up to 15 ft high. Travelway from the track to section goes through break 47. No injuries, entrapment or equipment involved. No ventilation affected.
A roof fall occurred in the #5 return entry of the I Submains that was approximately 10'L X 20'W X 7'H. The fall was discovered by the examiner and the MHSA inspector.

A non-injury roof fall occurred in the old works of the Northeast Mains Panel 36. The fall was discovered during an examination of the areas. 90'in length,18'in width, and 5' to 6' in height. The fall was investigated by MSHA officials.
Freezing and thawing temperature changes contributed to material becoming unstable. The effected area did not show any signs of weakness or deterioration prior to rock/material sliding off highwall into pit.
An Unplanned Roof Fall occurred on Cross-Cut 100 between Entry 7-8 Main West 2 Primary Escape way. The Roof Fall measured approximately 35' in length, 17' in width, and 5'-5 1/2' of thickness.
A roof fall occurred in the supply road of the 2nd main west - old, #1 unit, no injuries or disruptions to ventilation. The fall was approx 82' long, 18 wide, 8' high. The fall was discovered at approx 12:pm at xc79 spad 53 + 46.
The #1 hoist went out of service due to rope stretch. It was observed that the skip was unable to to reach elevations high enough to dump. The rope was taken up to to correct the problem. Hoist was placed back into operation at 12:00 am 01/19/2011. All miners were promptly evacuated through the #3 escape shaft after discovery of hoist malfunction.
An unplanned roof fall was discovered by the mine examiner. Fall is 1 x-cut outby spad No. 5646 approx. 4000 ft outby the faces. Fall measured 30'L x 20'W x 6'H. This area will not be rehabilitated.
Flint Ridge Mine #2 has experienced a non-injury un-intentional roof fall outby the 003 working section at cross-cut #42 along 2 South Main in #3 entry, (neutral).The roof fall dimensions are approximately 7 feet thick, 35 feet long and 30 feet wide.
A waterline leak was being investigated in our old mine 2 west mains. When the leak was found it was due to a roof fall at crosscut 81 in the old beltine.
An unplanned roof fall was discovered on old #1 section (old #8 Belt Panel) one break outby the last line of crosscuts. Fall was from #5 entry to #8 entry. Date of development was 6/2010. No injuries or entrapment involved. No ventilation, escapeway, passageway or equipment involved. Fall was approximately 115 ft long x 19 ft wide x 19 ft high.
The operator chose to take down the 8 Haulage elevator for more than 30 minutes for repairs.
At 5:15 the CO detector at #4 belt head went off, the belt was shut down. Two workers were close by & responded to the area. Discovered smoke, called outside & reported findings. All men were evacuated via the intake. After smoke was cleared & further investigating, a rock was found in the tail with coal causing a spillage & the belt to run off in tail piece causing heat & smoke.
The weekly examiner found an unintentional roof fall in the 8N bleeders in the #1 entry at the #30 crosscut. The fall measured approximately 18'W x 18'L x 12' H.
A roof fall occurred in the intersection 1 break out-by spad #944 in the # 3 entry.The fall measured approx. 19'W X 25' L. X6' Thick. Persons, ventilation nor equipment were effected. This area will not be rehabilitated.
A rock fall measuring 4'H X 20'L X 20'W was found at break number 75, one (1) break outby spad number 1305 in the crosscut between the number two and three intakes. This fall did not interrupt the air flow or interfere with travel.
An unintentional fall occurred on the second bench of the high wall between the return and portal entries. The piece of rock that fell measured approximately 25 ft long x 14 ft wide x 6 ft thick. In the fall two man trip barns and one 2 man personal carrier were damaged.
A roof fall occurred in the # 7 return entry of I submains that was approximately 20' X 20' X 5' in height. All entrances to fall have been breakered off. MSHA was notified.
MAINTENANCE DEPARTMENT WAS CONDUCTING THE SERVICE CAGE EMERGENCY GENERATOR TEST. THE SERVICE HOIST WOULD NOT RESET UPON COMPLETION. THE MINE WAS EVACUATED AND IT WAS DISCOVERED THAT A BAD ETHERNET CARD WAS AT FAULT.
The 8 Haulage elevator was down for more than 30 minutes due to the malfunction of a door switch.
A roof fall occurred in a neutral entry at the C-seals set. The fall was approximately 15'L X 19'W X 5.5' in height blocking access to one seal in the C-seals set. MSHA was notified and investigated the fall. All entrances to the fall area have been breakered off.
A roof fall was discovered in 5B long wall tailgate entry in crosscut adjacent to weekly airway at 106+00. Dimensions measured 16'wide by 12'high, length was unable to be determined due to inability to access.

An unintentional roof fall occurred at 75+30 in entries 1 and 2 in the 4H panel. The fall did affect ventilation because it took out a stopping between the intake and return. The fall did not affect passage of men or cause personal injury. The unit has been idle and is in the proccess of moving. The fall measured 100 feet x 18 feet x 4-6 foot high.
A non-injury unintentional roof fall occurred at the mentioned mine approximately 1000 feet outby the 002 active section. The fall occurred in a neutral entry (#4) in an old roadway intersection on #5 Main. The fall was app. 7 to 8 feet thick, 26 feet wide and 25 feet long. Laminated shale gave way which was supported with 60 inch fully grouted rods.
Oil inside of Kiln 5 bull gear housing ignited at 9:39am partially due to kiln being shut down recently. The fire was contained within the gear housing and extinguished using hand-held extinguishers at 10:20am. No injury or equipment damage occurred as a result of the incident. MSHA hotline was contacted at 10:12am to inform them of the fire.
An unplanned roof fall occurred at one break inby spad #1086 that measured approx. 50' L. X 20' W. X 12' H. No injuries were reported. The area was developed on 6/1/10.
The level switch on the 8 Haulage elevator malfunctioned causing the elevator to be down for 70 minutes.
Rock fall occurred on 2 East Mains in the #2 entry at break #11. Fall measured approximately 20' wide x 60' long x 5' thick.
One crosscut inby spad 1833 in the #1 entry adjacent to #5 head intersection fell 20' X 20' X 6'. Doesn't block travel way, timbered off contacted MSHA & State.
While drilling in a field, off mine property, a pocket of natural gas was encounterd and ignited. The fire burned for 30 hours. This 7000-1 is being submitted in protest of citation 8503166 and MSHA's jurisdiction in this matter due to the drilling was off mine property.
A fall of roof occurred in the Southwest Submain at 44 cross cut, # 7 entry. The fall measured approximately 20 feet long x 20 feet wide and the thickness is approximately 9 to 11 feet in depth. The area was supported with 6 foot fully grouted roof bolts.
A roof fall was discovered inby spad 23780 in the #4 entry a neutral, adjacent to 2A belt line, in the intersection. The roof fall measured approximately 19 feet wide, 20 feet long, and 10 feet high. No injuries were reported as a result of this fall.
A unplanned inundation of gases into the working area which requied the removal of employees from the affedted location until conditions were stablized.
Heated material came in contact with acetylene and oxygen cylinders. The heated material caused the acetylene cylinder to over heat and the pressure relief valve to break and a 2"" rupture in the acetylene cylinder, creating a flame exiting the cylinder.
After 1st midnight crew had entered mine, a problem occurred in the PC controller for the man cage. It would operate on manual, but not automatic. After 3 attempts, it was decided to evacuate the 16 persons underground and try solving the problem. Evac called at 11:45 and all outside at approx. 12:30. No injuries or other problems.

An unintentional roof fall was discvoered at 6 A.M. on 01-11-2011 on a pre-shift examination. The fall was located at break 63 between #5 and #6 entry. The fall measured approximately 18'w x 35'l x 7'h.
Fall was found 23 break, 1 west mains 7 right crosscut. Fall was 15' long x 5' to 5 1/2' thick x 18 1/2' wide. Roof support was 5' torque tension bolts.
A scoop was being hoisted out of the Enlow Fork mine slope. The scoop was secured to dollies and the chains broke at the 1200 foot mark. The scoop slid off the dollies and continued to slide to the 1800 foot mark, having jammed into a man hole. No injuries or damage to the slope hoist.

A roof fall was discovered on the #4 Mains Section (#21 Crosscut, #9 Intersection). The fall was 18' x 18' x 8'. The area was bolted with 6' Super Twist bolts and 12' cable bolts. No equipment, ventilation, or people were affected by the fall.
During a pre-shift examination prior shift start an unintentional roof fall was discovered along the #3 East Mains neutral entry beginning at CC31 and extending inby too CC33. The fall description was approx. 200 feet in length x 19 feet wide x o to 8 foot in thickness. Type of supports in the affected area was 5' fully grouted rods.
A roof fall was discovered on North mains #1 Panel. No equipment was involved and no interruption on ventilation. It was investigated by MSHA & State. Outby Spad 5510
An unintentional roof fall occurred 8 WHG MMU 009 inby c.c.#48 SS# 6540. The fall was approx. 30'L. X 18' W. X 9"" thick. The fall will be cleaned up. Fall # 28A
Plant personnel noticed increased seepage from d cell. Upon investigation a boil was noticed on the downstream slope along with a small sinkhole. Upon discovery it was immediately reported to MSHA as having a potential to be an unstable condition at an impoundment.
An unintentional roof fall was found at 8:10PM in return airway. The fall was between #3 and 4 entries. Approx 60' Long X 19' Wide X 6.5' High. The area was bolted with 4' resin and 6' torque tension bolts. Fall will not be cleaned up. Called toll free # @ 8:25PM on 01/08/11.
An unintentional roof fall was discovered on the Main South Travelway at 9:16PM on 1/8/2011. The fall measured approx. 20' W X 70' L X 5' thick. The area was developed in April of 2000, and supported with 4' fully grouted bolts. This fall will not be cleaned up.
The latch on the outside door at the bottom of the elevator shaft would not latch in order for the elevator to be operational.
An ignition of methane gas may have occurred in the No. 4 face. The continuous miner operator began cutting down coal in the middle of the face when a yellow flame occurred across and in front of the ripper head. The flame was extinguished with water and rock dust. There were no injuries or damage to equipment.
A production miner was attempting to tram the bolter out of the face from the basket when he lost hydraulic functions to the inch tram. Troubleshooting the possible causes he then noticed flames billowing from underneath the engine compartment of the bolter. The operator activated the fire suppression system and exited the basket.
During the first load of the shift, the end gate on an ash truck failed to open during the dump cycle causing the truck to tip back onto the end gate. This prevented the operator from being able to exit the truck. The operator was safely removed from the truck by 09:20 AM.
An unintentional roof fall occurred on the 4C belt line between 72 and 73 crosscut in the entry #6. The fall measured 40 feet x 19 feet x 6 feet high and will be cleaned up. The fall did not affect passage of men or cause personal injury. The fall fell on 54 inch rope belt structure.
Miner entered the elevator, when he pushed down button, a power failure occurred (power company lost grid). The elevator drifted upward 2 to 3 feet above normal stopping point. The elevator lost it's positioning and had to be re-set. Once the power was restored the elevator re-set and positioning was re-established. The elevator moved and the miner was able to exit. No injuries.
A roof fall occurred on the #2 unit in the intersection of #8 entry at spad number 45+10 and extending through 1/2 the pillar line between #7 and #8 entries measuring approximately 25ft. long x 18.5ft. wide x 8ft. high. This area was supported with 4ft fully grouted roof bolts and 8ft. cable bolts.
An electrical issue occurred at the override portion of the elevator. This issue made the elevator inoperable.
Rooffall on #1 Beltline top was heavily laminated in that area. Area was cribbed off to prevent any access. Production was stopped at this mine. Equipment is being pulled to the outside.
A fall has been found 2 breaks inby survey station No. 3387. This fall does not impede travel or affect the course of air flow. All access entries to this fall will be cribbed and dangered off. This fall measured 25' long x 19' wide x 3.5' high.
At approximately 12:20 pm on January 5 2011, an unexpected roof fall was reported to mine management. The location of the fall was 1 East return, #1 entry between 16 and 17 wall. There was no effect to ventilation, the size is 25' rib to rib.
A fall has been found inby survey station No. 3391. This fall does not impede travel or affect the course of air flow. All access entries to this fall will be cribbed and dangered off. This fall measured 20' long x 19' wide x 3' high.
At approximately 1:05PM, the slope car and hoist became inoperable. The drum brake on the Nordberg slope hoist would not hold. The hoist was taken out of service. The hoist was repaired.
Roof fall, D-Butt #1 & #2 entry. 3600 ft. outby face. 5 ft. high, 18-20 ft. wide, 80-90 ft. long.
Examiner on weekly examination of worked out area discovered area had heaved and caved since last examination. The area inby cannot be safely examined. The roof in this area had been supported with FG 4-foot resin bolts with 8"" x 8"" plates. Cable bolts (12' long)also installed in this area. Cave = 500' x 18' x unknown height). Area outby cave cribbed to prevent extension.
At approximately 12:05am on January 5 2011, an unexpected roof fall was reported to mine management. The location of this fall was #63 x-cut, mother #3 beltline in area 2 of the mine.
An unintentional roof fall was found on 3rd shift while examiner was making his returns. Fall measured 60'x19'x8' high. Fall affected ventilation. It knocked out part of a return air stopping. Fall did not affect passage of men or cause injury. Fall won't be cleaned up. The stopping was rerouted & ventilation reestablished. Location of fall was 31+50 entry #4 in the 4C return.
A roof fall was discovered in the Cross-Cut 12 between Entries 3 and 4 on the 1A Submain. The roof fall measures approximately 56' Long, 18' Wide, and 6' in Thickness.
An unplanned roof fall was discovered a break #29 of 1st NW Panel in the #1 Entry at spad #1213. Fall was approximately 75ft in length X 19ft wide X Approximately 12ft to 14ft in height. There were no injuries , entrapment or equipment involved, no ventilation escape way or travel way affected. Supplemental support was installed in area and fall will not be clean up.
A roof fall was discovered at 1:35 pm at crosscut 8 between # 2 and # 3 entries of 8 South. The fall measured approximately 20 ft. long, 20 ft. wide and 6 to 7 ft. high. The fall did not impede ventilation or escapeways.
Rock fall occurred on the 2nd left off 1 left off 1 South East Mains. Fall measured approximately 18' wide x 70' long x 6' thick.
An unplanned roof fall was found in an area where persons do not regularly work or travel. Between break #4 & break #5 of 1st NW Panel in #2 entry (spad #851 appr. 60' inby fall). Fall was appr. 70'L x 19'W x 12 to 14'H. No injuries or entrapment involved. No ventilation, escapeway, walkway or equipment involved. Fall will not be cleaned up, will be breakered & dangered off.
On 01/04/2011 a pillar fall occurred on the MMU 023-0 section. Water came from under the pillar gob and after two days accumulated in low areas and resulted in the secondary escapeway being blocked. MSHA determined that this was an ""inundation"". This report is being filed to avoid any further enforcement action.
#2 hoist ""kicked"" due to an e-stop from the cage. Investigation revealed that the trailing cable on the hoist had been damaged. There were still two means of egress to surface available to personnel underground.
During his weekly examination the fire boss found a roof fall approximaterly 20' X 20' X 6' in an intersection near spad 20261. Ventilation and travel was not impaired and the fall has been danger off and will not be rehabilitated.
During a routine inspection, a guide rope on the 16' production shaft was discovered to be damaged. The hoist was taken out of service for repairs. We still have 2 hoists available for use - the 14' hoist and the 8' hoist.
A roof fall occurred while retreat mining was taking place. The fall was located just outby the MRS units on the 005 working section. The fall was located inby crosscut # 54 in the #4 entry and measured 20' by 15' and was 5' in thickness. No equipment or persons were involved.
IGNITION: A piece of metal became entangled in face chain on #2 LW causing a spark at joint in the panline w/shield 144 causing the ignition. The ignition was orange and blue in color and went out immediately after it ignited. It lasted only seconds and was approximately 12"" in height.
A roof fall was discovered on the track outby 001 section @ spad 166 in the #2 entry. The fall did not affect persons, equipment nor ventilation. The fall is approximately 30'L x 30'W x 8'H. The fall will be cleaned up and an arch installed (upon approval of MSHA)for support.
A roof fall has occurred in the #4 entry between brk. 10 and 11 in the Hogg Mains primary escapeway. The size of the fall was approx. 20' x 7'x 70'. The area will be cribbed, timber and dangered off and the primary escapeway will be brought around the fall into the #5 main entry.
A unplanned roof fall occurred on the #2 section between spad #'s 1254 and 1255.Fall was approx. 12' H. X 30' L. X 35' W. No injuries or entrapment involved. No ventilation or escape way interruptions. The roof fall fell on the ends of shuttle car and section transformer.
A roof fall was discovered during an idle shift along the D-Main Travel way 1xcut outby sta #2376. The fall measured approx. 8-10' high, 20' wide, 25' long. The area was bolted with 4' fully grouted resin bolts.
Mine was idle and a foreman found the roof fall at 1:35 pm, at the intersection of station 2+40, along track South Mains.
An unintentional roof fall had occurred in an intersection and extended into the crosscut at crosscut 3 in entry 3 on the main east. The fall was approximately 20' x 30' x 12' high. The roof was bolted with 5' point anchor bolts, truss bolted and had 10' point anchor bolts in a star pattern of the intersection.
An unintentional roof fall has occurred on the 2nd Main South in between the XC-42 and XC-43. The fall was approximately 55 feet in length and 17 feet wide and 6 feet high. The fall was discovered during a pre-shift exam after the mine had been idle for the holidays.
The victim received fatal injuries and was found in the #4 belt head chute. There were no eyewitnesses. The circumstances surrounding and the cause of the accident are under investigation.
Employee was taking a left-handed lift on the #2 pillar section. He backed the miner out to check a leaking boom swing hose. He and a miner helper were on the offside of the miner checking the leak when a rib rolled and struck employee on his lower left leg. He received multiple fractures to the left foot. *Doctors had to amputate below knee.
EE was flipping a mantle with a bridge crane and strap when the strap broke causing the mantle to fall to the floor injuring employee.
Employee was climbing over a discharge belt and grabbed a V-belt. The weight caused the belt to move, his left hand got stuck in the belt and pullet, cutting off the tip.
Removing chain hoist, chain slipped down and caught finger cutting off tip of left hand ring finger. Employee began missing work as a result of this injury on 01/25/11.
Employee was cleaning the dust off of the roll bar while driving a 7 ton buggy. At heading 4014 straight, his right hand got caught between the roll bar and the back. Skin was removed from 3 fingers exposing bone. Bones were crushed in the palm of the hand and 3 and a half fingers were removed during surgery. Skin grafting will need to be performed.
Raising the head up to push bolt in the hole and the plate on the bolt slipped and caught finger. Severed Finger - Right hand middle finger.
Employee was moving a 26x18 slab of stone and felt a pain in his groin. Employee saw a doctor the next day. Suspected hernia.
Injured was working as a parts runner. He was loading timbers into the end of a Brookville man bus & one of them would not go in fully. EE climbed in & lifted the timber to move it back into the man bus & said he felt a pull on the right side. He saw a Dr that diagnosed a small hernia. He saw a 2nd Dr on the 26th. EE had not ceased to work until the 26th. To return 2/3/11.
The shuttle car started to leave the face area and as he trammed the car about 3 to 4 feet, the tires started to spin. During this time the injured employee stepped between the car and miner when the shuttle car backed up to try and free the tires. At this time the bumper of the car struck the injured employee's right leg between the bumper of the car and the bumper of the miner.
Injured worker was working on 107 pan conveyor and complained of soreness due to bending and twisting on steps while removing guards. Reported the next day that he was suspicious of a possibl e hernia. Appointment was made to see the doctor. Was referred to another doctor.
EE felt pain in his lower right groin area when he stepped off the roof bolter he was operating. He continued working until 4:00 pm, end of shift and worked the next day, his last day worked 1/6/11.
Bolting top and hand was on arm. When raised, finger got caught between pot and top of roof.
Miner was attempting to remove a bearing cup from a wheel hub using a hammer and a piece of 3/4"" keystock. Upon striking the keystock, a piece of metal flew off and imbedded in miner's bicep requireing outpatient surgery for removal.
As the employee was installing roof bolts a piece of rock fell and hit his head.
He was working, stepped on a rock, slipped, fell and then couldn't walk. He injured the Left Knee.
Employee sanding using a wire brush on a drill and wind blew metal into eye.
EE was standing beside the forklift as they were loading the scoop bucket with a bag of bulk dust. He reached down to get the pallet and had his right foot there also as the forklift pulled up, when he did this, he ran over his right foot causing a contusion with the forklift.
Employee started to back the continuous miner out of cut when the outside bit struck his foot.
Employee was removing a belt from a conveyor. Sand or some foreign material entered his left eye under or around his safety glasses.
EMPLOYEE STEPPED OUT OF A MINITRACT HIS FOOT GOT HUNG UP AND HE FELL ON HIS BACK.
Hit finger loading belt rails on a scoop.
Employee stated he had just installed a bolt when the top cracked and a piece of material fell striking him on the back and back of the left leg causing a sprain.
Employee was climbing down the ladder of a 365 Trac-Hoe and lost his footing. He fell approximately 4 feet to the ground injuring his back.
While employee was picking up a 4"" x 14' I beam with a second employee onto a lifting bracket the employee felt pain in his lower back. This was in 3N 2 entry XC - 19.
Employee was preparing to clean out traps of the washplant spirals when he fell down a trapdoor that had been left open by the previous shift
Debris fell from raise bore shaft creating massive air rush knocking employees to ground and assaulting them with particles of rock and mud.
While testing a nip on a 1000 volt Dosco, arc flash occurred causing flash burns to the employees hands and minor burns to the face. He was seen at the ER, given silvadene and dressing and referred to another doc. He was seen in Reno and told to remain off work until 02/19/11, no physical therapy required.
While trying to remove a bolt from the hydraulic fluid reservoir the wrench slipped and cut his right hand.
Member walked up and asked the miner man how much more he lacked so he could come across and clean that side of the section and the cut where the miner was fell in on the miner and a piece of rock slid down and hit member on the ankle. He was taken by ambulance to Raleigh General where he met Analabs for a drug screen and was scheduled for surgery on 1/29/2011.
Laborer was in the process of filling a 100 gal. auxiliary fuel tank situated inside the mine's service truck. Unbeknown to him at the time, the truck's engine compartment caught on fire and spread immediately into the trucks mid section, causing what appeared to be a minor facial burn and smoke inhalation.
The contractor employee had cut the metal bands from a bundle of crib blocks. The contract employee started to pick up a crib block, when he tripped over an cable bolt causing him to fall. When he fell he hit his left ear on the loose metal band causing a laceration to his left ear.
Ee was dismounting backhoe and suffered extreme pain in groin. Went to Dr, who said he had a hernia. Went to a surgeon , who said he had a hernia. He worked until 2-3-11. Had his surgery on 2-4-11 and is now off work for a while.
Driver was climbing out of his truck backwards and stepped down with his left foot. He twisted his left ankle and he felt a sharp pain. Driver fractured his left foot and ankle in 2 places.
Employee driving a mantrip in the outby area of the mine. While traveling through a tight area a pogo stick that was on the back of the mantrip came in contact with a ventilation control, caused the pogo stick to spring around hitting employee in the face. Employee received multiple fractures to the face area including nose, orbital bones, and upper part of mouth.
The individual was roof bolting and had finished drilling the hole when the drill steel fell out of the hole hitting his left hand causing a fracture to the ring finger.
The employee was traveling through a mandoor and twisted his ankle. He did not miss work until 2/9/2011. The event has resulted in a lost time injury.
Rock fell from back into basket of JLG, hitting ee's left leg and pushed knee into machine. The heading had been scaled prior to ee entering the area.
Employee was riding the mantrip underground when the rear tire of the mantrip unexpectedly dropped into a hole in the roadway. The employee was sitting in the rear of the mantrip and was bounced up off the seat and struck his head against the roof of the mantrip causing injury to his neck.
The employee was riding the mantrip with the section crew at the end of his shift. As the mantrip traveled through a ventilation curtain, the miner trainee raised his left leg to hold up the curtain. The employee's leg was caught on the curtain board resulting in a hyperextension of his knee.
Subject was working on Northwest 180D shovel bucket when he slipped and fell across a block of wood on his back
Alleged that while operating a mantrip his SCSR or hammer caught a timber and drug him off the man trip. Investigative facts do not support what happened.
Dredge was being pulled to bank by the excavator. While the dredge operator was allowing the far cable to give slack a miner who was in the aluminum boat allowed himself to get near the far cable and he states that it struck him. No witness.
A panel of mesh wire (screen) had stuck to another piece, the other roof bolter operator jerked on wire to free. When wire camed loose it had an upward movement and struck the injured employee in the face, knocking off safety glasses and hard hat. The result was a scratch to the left eye.
Employee just got done refueling the blade. He had the nozzle in his left hand and had a hold of grab rail with his right when his foot slipped and all the weight was distributed to his right arm, thus straining the shoulder. Employee was following first aid measures but on 4/27/11 it was decided he should be evaluated by a clinic.
Employee stated he was moving a 6""x6""x8' timber out of the roadway, when he picked up the timber he felt pain in his lower back on right side and had swelling on his abdomen. The EE did not miss work until 2/17/11 and became reportable at that time.
While exiting the skid loader that the ee was operating he slipped on the bucket and sprained his lower back. Although the injury occurred on 1/27/11 it did not become reportable until I received the DR's paperwork on 1/31/11.
He (the injured) was helping the welder providing him metal rods to weld a part. When the welder finished he started to clean the part and from the dirt that came out, a piece of approximately 1/2"" hit him in the right eye causing him a laceration.
EE was hauling shot rock when another machine operator approched EE went to his right and hit a frozen pile of rock which was covered with snow causing the truck to shift to the left. This caused the EE to hit his head and leg.
Employee was sledge hammering when the wedge flew out and hit his cheek bone causing a laceration to the face. The employee was wearing safety glasses! All employees have been notified that they MUST wear full face shields!
Employee was exposed to spilled Hg while cleaning a spill around scrubber unit. Biologic tests showed a high amount of Hg in this miner's system. Miner was immediately removed from area. Placed under medical care to reduce Hg levels further, she was removed from site by doctors advice and given medical to help assist in the reduction Hg levels in her system.
When cutting out at tailgate shearer drum threw a rock striking employee on the right foot.
MECHANIC WAS CHECKING SQUEALING NOISE ON JOB FOREMAN TOYOTA PICKUP. HE WAS SPINNING THE FRONT RIGHT WHEEL WITH HIS LEFT HAND. WITH HIS THUMB ON THE RIM HIS GLOVE CAUGHT SOMETHING ON THE BACK AND PULLED HIS THUMB INTO ONE OF THE HOLES ON THE RIM. HIS THUMB STOPPED ON THE BRAKE CALIPER AND THE WHEEL KEPT TURNING. HIS THUMB WAS MASHED AS A RESULT.
Employee had just pulled the master key out of the 990 loader. He was starting to walk toward the truck and turned his ankle on unlevel ground. The ground was very wet due to weather conditions so when the 990 parks it leaves little ruts causing unlevel ground.
Injured was taking down 1.25"" plastic line from mine roof when he cut the ties that held up the line it fell striking him in the top of the head.
Employee was a passenger in a Genco mantrip, when traveling up the mine slope out of the mine, when they collided with another Genco mantrip. Employee was struck against an unknown object, resulting in a strain/sprain to his back. Employee went the next day to Dr. where he received treatment.
The employee was helping load pin supplies on the back of the bolter when his left hand got hung in a empty pallet that he was trying to throw on the back of the bolter, causing him to hyper extend his left arm.
Employee was nailing concrete forms together while in a kneeling position. When he stood up, he said he had a sharp pain in his lower back.
EE is employed by J Rayl Transport. On the day of January 26th 2011 he was running a normal Morton Salt Shuttle. EE was getting out of his tractor to check the trailer and slipped on wet steps. When EE fell, he fell backward and hit his head. This was around 6:15 am. The wet condition of the steps contributed to his fall.
While jacking up the front of the tailpiece, the jack flew out striking him on the side of the face.
Loosening bolts on feeder when he felt pain in his right arm. Late report 1/28.
While welding on a bucket adjacent to another welder the employee was affected by the arc of the other welder. The employee's eye was scarred.
Employee was operating shuttle car and was turning into crosscut from #2 to #3 entry. He had his left arm resting on side rail of car and caught his arm between shuttle car and rib line.
He had put bolt in hole and was putting bolt onto wrench. Then stated that coat sleeve had got caught between the bolt and wrench, he began to spin the head to align the bolt into wrench, sleeve became caught between the 2. it caused his right arm to twist around the bolt wrench. The ee continued to work until 4/27/11, he then began missing work due to injury.
Poor winter conditions, snow covered ice, sub-freezing temperatures. Employee slipped and fell on left shoulder.
Injured was walking around to the front of a twin head roof bolting machine when he slipped on loose coal spillage and caught himself on the tray of the roof bolter. When installing subsequent roof bolts he felt a pain in his right shoulder. Employee finished the shift but has not returned to work.
EE was pulling a water hose across the cat walk back into the plant and stained his right shoulder.
Employee stated that he went to kick battery breaker off to lock and tag out for next shift. It was hard to kick off so I pushed real hard, something popped and burned in my left shoulder. Employee later went off work to have surgery. While off work, he had surgery on both shoulders.
Employee was walking to get supplies when he stepped in a hole and strained his right knee. Employee started missing 03-02-11.
Employee was trying to remove the cover off number eight silo air slide. During this time is when the employee felt a sharp pain in his lower back. Diagnosis, were lumber strain.
EE was roof bolting and had finished drilling his next hole. Roof conditions were lower than normal and when he tried to install the 6' bolt, he could not get the bolt far enough in the hole to push the bolt up with the drill head. He attempted to push the bolt up by using the roof bolt plate, the plate slipped and struck him in the right knee.
EE raised the engine compartment door located on the side of a rubber-tired loader to work on the engine and before he could secure it in place a wind gust blew the door over onto his arm.
EE was working at the face in 18 W-40 blk, turned to pick up one end of a 10' X 20"" ventilation tube to hand to another EE when pain was felt in his lower back.
Employee got out of his personal vehicle to go to work. Employee fell on ice as he was going to start his loader. Employee received pain in his back, neck, head and left shoulder. Employee treated at ER. Was to return to work immediately. Then he went to a chiropractor on his own. Chiropractor said he was to be off work from 1-25 to 1-31-11. Employee returned to work on 1-31-11.
Employee had left his scoop bucket out in travel way. A coal hauler came along and struck his bucket and knocked it around into the rib. He was between it and the rib. Broke bones to sacrum and tail bone areas.
Employee was removing relay bar pins on the 15D longwall tear down using a hammer and felt pain in his right shoulder. Employee was diagnosed with a rotator cuff tear. No problems with mining methods, machinery, training, PPE or compliance issues.
Employee was in company #58 portal bus while attempting to push company #53 bus was off track through a switch. While doing so the injured stated he struck his lower back on the window of the portal bus, causing a strain to his back.
Employee stated that he bent over to pick up the miner cable, when he felt a sharp pain in his back. He worked the remainder of the shift and decided to go to a doctor the next day. The doctor referred him to a specialist.
Employee was driving haul truck, and placed his hand under the arm rest to open the window. At that same time he struck a pothole shifting him to the opposite side of the truck, jarring the left arm and shoulder resulting in left shoulder sprain.
While dismounting from an end dump truck, the injured stepped on a dirt clod or piece of shale and turned his ankle causing a strain
Employee was attempting to lift Joy 21 shuttle car tire up to roll it into a scoop bucket. While lifting the tire he felt a sharp pain in his lower back.
Employee noticed pain in his right knee before his break at 6:10 pm. When he went back to work after the break he stooped over to stenciling a sack of material and felt a pop in his right knee. After that he had excruciating pain in the knee.
At the beginning of the shift the employee tried to climb into his haul truck but slipped on the ladder falling, and hurting his back.
He was getting a tire for the man trip when he slipped and fell (on ice). He did not cease work, he went to the doctor on 2/2/11. He still worked until 3/22/11 (that was his 1st day off work). He went to the hospital and they operated on his left shoulder on 3/22/11.
EE was handling scaffold material overhead at #4 Alumina Load Out Track when he began to feel soreness in his right shoulder and between his shoulder blades. On February 23, 2011 he was given a Cortisone injection, making this case a Medical Treatment.
Employee was lifting a miner cable when he strained his back. The employee continued to work until May 23, 2011 at which time he was removed from work by his physician. This accident has been under investigation and has now been determined (June 17, 2011)to be work related and reportable.
Employee was walking across a patch of ice which had snow on top of it. He lost his traction and slipped and sprained his left ankle. He went home and his ankle had swollen up. Then he went to the emergency room.
The water pumper stood on the tire of the water pump to check the fuel level and slipped off the tire and landed on his left elbow, bruising the elbow. He did not report the incident until 01/24/2011.
Employee was moving tool cart with another employee. While moving cart, cart slid sideways smashing employee's left hand against a FCC screen.
While working on a panel for the surge belt he walked into a low hanging brow, jamming head and neck. ****He continued to work full duty without restrictions until he started missing work on 05-23-2012 due to surgery.****
The injured employee was in the process of straightening out a roof bolt, when the bolt spun, causing him to strain a muscle in his back.
While walking in surface screen area, employee slipped on the ice, landing on his right ankle. The bones above the ankle were fractured from the FSL.
A GATE FELL OFF ITS ROLLER AND LANDED ON OFFICE STAFF. IT HIT HER HEAD AND SHE SPRAINED HER RIBS AND BACK.
Injured stated that he tried to loosen radiator cap on 100 ton locomotive, when hot steam and liquid contacted right wrist, causing burn to wrist area. Injured was in process of checking engine coolant level because engine was overheating.
Employee was operating a haul truck and was being loaded by the 02 shovel. The 02 shovel bucket made contact with the haul truck jarring the employee in the haul truck cab, resulting in a low back sprain.
Employee was cleaning rollers, inside the die. Glove got caught. Injured fingers on his left hand
EE slipped and fell on ice while getting into a company vehicle. He fell on his buttock and back. It caused such a bruise that it set up an infection. He had to be hospitlaized and surgery was performed to remove the infection. He is now doing well and should be back to work in a few days.
Pumped fuel out of fuel tank when tank was empty, it fill with fumes, last thing I remember I turned the pump off.
Employee was helping carry a steel beam when he pulled his groin muscle.
Employee states he has pain in his left shoulder, elbow and wrist which he feels results from repetitive work such as putting cribs in the tailgate. EE was sent for an evaluation and it has been determined he meets the CO Workers Comp Guidelines for an OI.
Employee was roof bolting when a rock fell from the roof striking employee on his lower back.
Employee was unloading a s/car tire from a scoop. He leaned the tire against the rib & was walking away. The tire fell & struck him on the left leg.
Employee complained of headache and feeling sick after welding in the kiln while bobcats were mucking the kiln.
14 left most outby fire-tee; employee was installing an extension on the bottom of the fire-tee. The extension was being installed on the bottom of the 1 1/2 inch shut off valve. The assembly blew off and the extension hit employee in the face and knee.
Employee was putting up screen wire in our Main West travel road with a double boom roof bolter. He missed the hole when trying to insert the roof bolt and the bolt bent. He then accidentally hit his rotation, instead of the lowering lever, causing the bent bolt to swing around and hit his right wrist.
EE was operating a spader (60 lb) when the bit became stuck. As he attempted to remove it, he felt pain in his lower back.
Employee's cap light cord was caught on a roof bolt and pulled his head against the top hitting his head on the canopy. This is to abate violation number 8154317
Employee hanging water hose and stepped off ladder onto a rock causing him to twist his ankle.
The employee was removing an intercooler from our haul truck (Unit # 232) and the cooler slipped off the mounting brackets and fell on his finger.
Employee was cutting a handrail off a barge when it fell and struck him on the back.
Employee was standing behind the miner operator when a rock 8 inches by 8 inches rolled from the rib stricking employee on the right side.
While replacing handrail the rail shifted smashing his middle finger on his right hand. Injury resulted in stitches to his finger.
Employee was walking down walkway. He slipped and caught himself with his hands and hurt his left shoulder. EMPLOYEE DID NOT START LOSING TIME UNTIL 1/31/2011 DUE TO INCREASED PAIN AND PENDING SURGERY.
He came down the steps from the crusher to fast and caught his foot on bottom step and fell onto a rock that was there and struck his leg.
Area of walkway had two timbers covered in dust that was extended under the track. Employee tripped over the first timber and struck his knee on a impacted rail.
Employee ""ALLEGES"" that while hanging a miner cable some foreign material (rock dust) fell into his right eye. Subsequent investigation revealed the employee actually had rust in his eye from an off the job accident. The employee's workers compensation claim was denied based on this fact.
EE was operating a air powered jackhammer. EE was removing built up cement debris. A solid piece propelled toward his face and somehow got behind his safety glasses.
Employee stated that while getting off a rock truck, he slipped and fell. He caught himself with his left arm causing his shoulder to be stubbed.
While pulling a rib drill out of the locked position, Employee inadvetenly hit the rotation lever for the drill which caused injury to his left little finger
Employee was taking garbage to a dumpster and slipped on ice. Bruised his right shoulder.
Injured was roof bolting on the #4 MMU and stepped on a piece of rock awkwardly and twisted his right ankle. Injured went on working the rest of shift and when he went home ankle was still giving trouble to him. Went to hospital and has a fracture in his ankle.
Employee was walking with a slider rail and rolled his ankle while stepping over a block.
EE was gathering stone and dropped a piece of stone on his toe.
The man was bending a roof bolt while pushing it into the roof when he felt a pain in his shoulder.
EE was stepping down stairs when he rolled his right ankle.
EE went to close gate slipped while pulling on handle, fell 15 feet to belt then 5 feet to floor.
Employee did not report the incident until 2/2/2011.The employee went to the doctor complaining of respiratory problems.The doctor asked if he had been exposed to any chemicals and the employee said ""another employee was cutting a bearing out of a pin bore when the grease caught fire.When a fire extinguisher was used to put out the fire, he breathed some of the chemical"".
EE was barring track onto dozer, felt discomfort in lower stomach. He thought it was pulled muscle and waited a few days to see if it got better - it did not. He went to doctor on 02/27/2011 to confirm it was a hernia which will require surgery. No lost time as yet.
Miner was standing on the third rung of a step ladder performing maintenence on a screening plant when miner lost his balance and fell backwards onto his left side in the dirt and mud. Miner suffered a concusion, seperated left shoulder, strained neck, strained back and ligament damage to his right knee.
Employee was closing the doors on a container above his head and felt a sharp pain in his lower abdomen. After visiting the doctor, it was determined that he aggravated a hernia that was not properly repaired. Employee had surgery to repair the hernia on 3/01/2011 and is scheduled to return 4/01/2011.
While working on a front end loader the hood fell and struck the EE on the foot.
While performing clean up in the finishing screens building at the tertiary plant employee had some material fall from above and hit him in the head and left shoulder.
Employee bagged 50 bags at 50# each on a pallet. He said on his last bag he felt a sharp pain from his groin to his stomach. He went to the emergency room. He is excused from work at least until his next doctors visit on 1/21/2011.
Employee was hanging miner cable on hooks during a powermove and sprained his ankle while trying to pick up more cable. Employee off work until 1/28 (working two days) and now is back off work until further notification
Employee was leaving work when wind blew lime in eyes.
Operator slipped and fell while exiting a CAT 771-D haul truck.
An EE was running the scoop and scooped up the pressure pump in order to move it. The door on the scoop deck was not shut and injured EE went to shut the door. Other EE thought that IE had completely moved out of the way of the scoop, but he had not. Other EE began backing up the scoop bucket and hit IE and pinned him against the right rib. He sustained injuries to his knee.
He was climbing down ladder of end loader, when he slipped and fell to the ground. He strained a muscle in his lower back. He was originally released to return to work on 1/19/2011, but he returned to the Dr. and has not returned to work yet.
EE stated he was in the process of lifting a acetylene tank out of the cart when he felt pain in his lower back.

While conducting fireboss run on Sycamore #1 belt this morning, member walked through a crosscut at 188 break and stepped in a rut in the bottom. Member heard a loud pop in his left foot and could not put much weight on it. He managed to drive the mantrip outside and transport himself to the Raleigh General.
Employee was lifting a crib block on uneven ground that had some thin ice in the surrounding area. The ice broke loose causing employee's foot to slip through resulting in a pull to the left calf muscle.
Crew was welding 18"" steel pipe, laying the pipe on surface as it was being welded. EE started to enter the interior of his welding truck, the pipe slid against the open door pinching his left leg in the door.
While climbing a conveyor ladder the employee's foot slipped off and jerked his back when he caught himself with his arms.
Slipped on ice that was on the gravel on the south side of the fire truck garage while walking to the pickup.

The employee lifted a lid off the 20's bin and felt a pain in his back. Injury was a sprain/strain. This was not a reportable accident until January 21 when the employee missed work.

With lingering soreness the employee had an MRI on May 31 revealing a rotator cuff tear & surgery was scheduled for June 16th. The employees 1st scheduled missed shift was June 17th.

Employee was moving a forklift fork with his right foot. The fork moved suddenly and employee strained his right knee. He has been on full work status with no restrictions since the accident. On 5-23-11, employee decided to have surgery on his right knee at which time this became reportable.

Employee was removing a 60"" top roller 2 bks out by #3 tailpiece. He had raised the belt with a chain ratchet and was pulling the top structure out when his back started hurting. He finished his shift then called in the following day stating he was going to the doctor who prescribed him physical therapy for 2 weeks because of a strain.
Employee was assisting 3 other miners move Grizzly component in place. While maintaining a vertical position on the component to rehook, the component became unstable and shifted. The employee's effort to maintain it upright failed and it leaned, pinned his elbow against the rib, bending back and breaking left wrist.

On an icy day the loader operator started his loader and climbed down. The operator slipped and fell on ice. The manager helped him up the operator was okay. In August the employee reported pain associated with the fall, after being diagnosed with a degenerative issue claiming the fall aggravated his condition. September 15, 2011 employee had surgery to repair his condition.
While moving items into the new parts room he felt a pain and burning in his groin area.
Employee had loaded concrete block in scoop bucket during the trip to face a block fell out of bucket and scoop ran over it, causing it to drop down suddenly.

He was diagnosed with a possible pulled muscle and given two days off. Returned to Dr. complaining of left arm pain and was told to get an MRI, employee off until given an MRI.
EE was checking oil on haul truck. Dipstick is hard to pull due to vacuum effect. She slipped when it broke free and fell on right hand/shoulder. Seen by a Dr. and x-rays showed no fractures. She got a full duty work release and returned to work that day. She continued to experience pain and saw a specialist. MIR revealed torn rotator cuff, she had surgery on 2/23/11.

EE was walking on refuse walkway up to the refuse bin. He was pinging the rollers with a hammer when the hammer got caught in the belt and pulled his hand and arm between the belt and roller. Trauma/compression injury left arm.
The employee stated that while hanging miner cable with 7 other employees, he injured his left shoulder. The incident became reportable on 3-5-2011 when the employee began missing work.
Cold front moved through the area re-freezing the surface of the road. Slipped on the ice twisting leg.
Worker was adjusting frames, he tripped lost his balance and fell on a frame.
All members plastered stoppings during the move and EE complained after the move of his hands being irritated by the b-bond. It had worn a hole in his gloves and gotten on his bare hands.
EE was helping the powder crew load a hole. As he was putting a wet bag in the hole, he felt something give in his groin. The diagnosis is hernia and he will have to have surgery to repair.
Pinched finger between two pieces of curbing on conveyors.
EE was putting up silt fence along the berms beside the Prep Plant. He slipped and his leg went under him and he landed on his left knee. He complained of a burning sensation in his knee, so he was taken to the medical facility for treatment.
The injured employee was in the process of putting his cable bolt wrench into the chuck of the bolter, when a piece of rock struck his left arm. This resulted in a fracture to his left wrist. The employee began missing work on 1/14/2011.
Party had got of his truck at the start of the shift to put a 6"" pump line back together. It was down in the twenties temperature wise over night. The water on the ground froze. As he went to put it back together he slipped and fell injuring his toe.
Employee alleges that he was performing work while kneeling on knee pads. When he stood up he felt a cramp behind his left knee. Employee was able to return to work with normal duties. His injury became reportable as lost time on 2/4/11.
Employee stated that he was getting out of his mechanic's truck. As he put his feet on the ground, they slid out from under him causing him to fall and strike his head against the floor of the truck. This resulted in a cervical strain and headache.
An employee was working in the mobile equipment shop and had mobile equipment running while the garage door was closed. This is against company policy/procedure. The employee suffered minor carbon monoxide poisoning. The employee never lost consciousness. The employee has since missed three days of work and has not yet returned. The employee has 35 years of mining experience.
EE was pulling a pin from the rail roller of the 951 miner. As the pin came loose the tail roller fell and caught his left thumb between the tail roller and the machine, causing a simple fracture and a laceration requiring 3 sutures.
The truck's wheel struck a pot hole and the operator strained his lower back.
Employee was working at the V-Wiper and turned and tripped over a block and fell twisting his ankle.

Employee was walking down a snow/ice covered road and fell backwards onto the hard surface on his back. His back was injured. However, he finished his work/shift and did not realize he was injured until Saturday 01-09-2011.
While employee was boarding the transportation van, the employee slipped on the door rocker panel causing the employee to do the ""splits"" and falling backwards.

Member was lifting come alongs above head to put into monkey face in order to bring a wrecked flat car over and onto track rail. While lifting he experience a ""pop"" sensation in back, he immediately ceased to work. He came outside and reported injury.
Employee is a pumper and was loading a five hp pump in the back of a truck. He felt a strain in his right shoulder and bicep. He continued to work but his shoulder and bicep still bothered him. He went off on surgery to repair a tear in his bicep on 3-17-2011 making this MSHA reportable.
Employee was exiting a 993K loader, when he reached the bottom step he slipped jumping to the ground, resulting in strain to the neck and lower back. (Employee continued to work until 1-14-2011).
EE,stopped to open airlock doors on track. He slipped and fell, hitting his right forearm on the rack rail. He was wearing rubber boots and the tread could have been worn. He continued working. He later went to Med-express where he was diagnosed with a hairline fracture to the radius soft cast applied and he has not missed work.
While employee was checking supplies on bolter, rock about 4' wide, 3' long, 8"" to 10"" thick fell out between bolts hitting the left side of his body; including his head, face, neck, shoulder, arm and hand.
Subject alleges that he was lifting a scoop tire trying to stand it up. He felt pain in his lower back.
Electrician was taking lead from a de - energized circuit breaker. The circuit breaker flashed and burned the electricians hands.

Employee was in the process of cleaning off the top of the continuous miner. When he lifted a rock to remove from the miner he felt a pain in his left shoulder. The employee finished his shift and sought medical attention after work. The employee is currently off work awaiting diagnosis.
While spraying degreaser onto roof bolter, with a pump up pressure can, the hose blew off can connection spraying degreaser onto the right side of face and underneath his glasses.
Employee was operating shuttle car when he ran into rib and bruised his left arm.
Employee tried to remove debris from conveyor without shutting off line. Glove became caught in roller causing a portion of employee's left thumb to be cut off.
During the drilling for ground support, while holding steel with left hand and the jackleg in an upright position with his right hand, employee pushed drill steel up, and immediately felt pain on his left shoulder. Employee did not miss work and incident was not reportable injury until surgery, employee's first day away was 4-11-11.
EE was plastering stopping when a few of the cinder blocks fell on him.
On 1/5/11, employee was exiting the CAT 385 and upon stepping from the top step to the second step, it was pushed into the top step. Right leg went to the ground and left leg was stuck on top step, twisting left knee. Injury was reported on 2/23/11. Received Medical Treatment on 3/2/11.
EE was attempting to install a long flanged pipe with a 1/2 impact. The impact did not work, so the EE used a right angle impact to finish the job. As the EE tighten the bolts the impact swung away from his body and jerking his arm. Surgery on 08/23/2011
Employee notified company on 10/28/2010 that he had been diagnosed with a triple hernia by his physician. There was no incident & no indication that injury occurred at work. At the request of the employee, the claim was filed with Alaska Workers Compensation and they determine don 01/05/2011 that the hernias may be attributable to his work because he has a physically demanding job.
Operating a CAT 777 Haul Truck when for some unknown reason, she stopped the loaded haul truck on an uphill grade placed the truck into neutral gear and allowed the truck to coast backward three truck lengths striking the berm causing the truck to roll over onto its passenger side. She was wearing her seatbelt and was not ejected. Accident contributed to ""Operator Error"".
EE states he was greasing equipment when a piece of rock fell and struck him on his head and right shoulder.
EE was working on the dust belt tail pulley, when the slate bar slipped and he hit his knee on the adjustment all thread. He continued working until the shift was finished.
Lifted kerosene heater - felt sharp pain in left side and groin area.
Enployee was carrying two buckets of miner bits when he tripped over a pallet and strained his lower back. Employee did not start missing time from this injury until 01/27/2011.
Member was bending and pushing bolt into top and heard his shoulder make a loud pop. He did not cease work but told his supervisor about the injury but didn't fill out an accident report. After the shift, he transported himself to Med Express in Beckley. Marfork Safety Department was not made aware of this until after he had seen a doctor.
EE was working on an engine on unit 1322 lube truck when and was pulling on wrench and his right shoulder popped. EE didn't go to doctor till the next day said that it hurt more the next day. Sprained right shoulder. EE worked on the 1/6/11 and 1/7/11.
The injured employee was installing a roof bolt. While in the process of straightening out the bolt he felt discomfort in his left upper rib area. The injury resulted in a fracture to his rib.
Employee was walking on the wet concrete walkway in the feeder tunnel when he slipped and fell on to the concrete walkway. Employee stated that he fell on his left side of his body, using his left hand to cushion his fall.
EE was walking when he stepped on a rock and rolled his ankle. EE was taken via ambulance to a local hospital and diagnosed with multiple fractures of the ankle.
EE was climbing down the ladder of a Cat 988B loader when the last step of the ladder broke. This caused him to pitch forward and bruise his chest on the ladder.
Member was using a 100 ton ram to straighten out a piece of 3/4 inch metal when the jack broke causing the metal to shift hitting him in the ankle.
Employee said he's had Dr. appointments since 2007 due to back pain. He began missing work from 1/4/2011 to 1/25/2011. Notes from his appointment with MD Orthopedic Surgeon on 1/25/2011 says employee has congenital stenosis and that there is no acute injury.
Employee was at shield #150 on the longwall when he stepped and twisted his right ankle.
Employee stated he was attempting to align the belt using a pry bar. While prying the belt over, the bar slipped and the injured employee fell back onto the rib and mine floor. He felt pain in his lower back radiating down his leg.
Employee called the morning of Wednesday, January 5, 2011, said his ear was badly swollen, that he needed to see doctor. He stated around 9-10am on Tuesday, January 4, 2011, he noticed pain to ear while working near dam generator. Doctor suspects the employee was victim of a spider bite.
Party was climbing down his haul truck. When he got to the last step to step down, his foot got caught causing him to fall backward on the ground with his foot still caught.
The employee was cutting a header hole when a piece of a miner bit struck him above the right eye and embedded under the skin.
Employee was unloading rockdust from the scoop, while unloading the rockdust from the scoop, he stepped backwards into a roofbolt causing him to strain his right shoulder.
EE was stepping off the back of a walk through roof bolter down onto the ground when he stepped onto a rock with his left foot. The rock suddenly moved under his feet causing him to immediately shift his weight to his left leg, resulting in a fracture to his left ankle.
EE called another EE and said he had gottin the truck out of gear and could not get it back in gear and he had put the truck into a ditch. (Driver error shifting on hill)

Later EE went to the Doctor and was checked over.
The operator dropped the white belt conveyor tray interrupting the proximity switch to remove a bag that was jammed. When he reached in to remove the bag the valvomat dropped down onto the operators wrist/hand pinning his hand between the valvomat and the white belt conveyor tray.
Employee was walking to his ride, tripped over a board buried in rockdust and fell on his right side, falling on his self-rescuer. Lost time injury.

Driver unloaded product hose to start unloading crushed lime, he was connecting 4 inch cam lock fitting to silo and his back started hurting. He was able to stand up with help holding on to silo leg. Mine EMT checked him out and sent to ER. The e.r. doctor said it was muscle spasms and gave him a muscle relaxer and 24hrs bed rest with a follow up visit on the 24th.
employee was in the company shop and was lifting a tractor tire to mount on tractor and felt pain and strain in his groin.

Employee was rolling out tensar on the corner of intersection with two other employees. A section of the rib, 62"" long, 12"" thick and 2"" wide rolled off striking employee on his left lower leg, causing a compound fracture.
Employee strained his right knee when he turned around to change seats in his shuttle car. He continued to drive his car until 1/31/11. He worked under restricted duty on 1/31/11 and 2/1/11. He missed work on 2/2/11 and returned without restrictions on 2/3/11.
EE was working in north faces left side. EE turned to walk toward the face when he stepped on a loose piece of coal and twisted his left ankle. Results of MRI on 1/29/11 showed a fracture to his left tibia.
While working on a tractor and climbing down missed a step falling about 6 inches and landing hard on his right leg. His right knee gave out and he felt a sharp pain. X-rays revealed no fractures or dislocations and he was released to the care of his own physician. Began missing work 1/24/2011 after an MRI revealed a medial and lateral tear of the right meniscus.
Kneeling on floor sliding crusher liners, when he felt a pop/sharp pain in his back.
Injured employee was using a sledge hammer while repairing a crusher, and he felt pain in his shoulder. After several weeks, the pain lessened, but did not stop. A second medical exam and MRI revealed 'impingement' of the rotator cuff, and surgery was recommended. The surgery was completed on May 20, 2011, and he returned to work the next scheduled work day.
Employee was unhooking spare floating pipe that was tied to the main line of pipe. While taking rope off of pontoon bit the pipe shifted, tightening rope and both hands were pinned between rope and bit.

Reported on 1/14/11 to supervisor but no doctor visit. 3/4/11 seen by doctor, heal with rest. 7/8/11 follows up, persistent popping. 8/11/11 seen by Ortho without reporting. 8/16/11 discussed proper procedure for doctor visits, first notice of physical therapy.
Service Tech. was atempting to climb up on a dozer, as he grabbed the hand rail his hand slipped off the rail, resulting in the employee slipping backward and falling on the ground landing on his back.
EE was placing 36"" belt structure on a skid. EE pinched left hand between skid and piece of belt, resulting in a cut to his middle finger. EE worked until 3/3/11. Left hand requires surgery to repair severed tendon.
