While the hoist man was performing his preoperational checks the hoist tripped, which sets the brakes on the hoist drum. This was determined to be the result of a power bump from our power source which is provided by Avista Utilities. The system functioned as designed when there is a power fluctuation. The hoist functioned normally when we located an electrician to reset it.

An unintentional roof fall has occurred in #3 Section Ellis Creek Sub Mains #2 Entry 2 breaks outby spad #12867. The roof fall was approximately 30ft x 20ft x 6 to 8ft. There were no injuries reported and no equipment involved.
Main Power Outage. All miners evacuated with no injuries or issues. Power was restored by 8:30 AM.
The small two man elevator was traveling from the bottom to the top when it stopped approximately 200 feet from the top due to the power cable getting damaged.
Employee was operating a D10 T Dozer, when smoke started coming into the cab of the Caterpillar Dozer. He turned his key switch off, got his fire ext. & exited the dozer. He used 3 fire ext. trying to put fire out, but could not ext. the fire. After an investigation of the incident it appears to be an electrical fire, that has destroyed the D10T Cat. Dozer.
Removed the #8 elevator from service at 10:45 am. Replaced the govenor rope. Tested and returned the car to service at 1:25pm.
Roof fall 70 feet inby crosscut #13, #1 entry, 9th Right Section. The fall was approximately 16 feet wide, 17 feet long and 8 feet high.
WHILE CUTTING COAL IN THE #1 ENTRY INBY #23 CROSSCUT ON THE E-26 SECTION AN IGNITION OCCURRED AT THE FACE. IT WAS ORANGE IN COLOR AND LASTED SECONDS. NO METHANE WAS DETECTED. MINING STOPPED AND THE AREA SECURED. MSHA WAS NOTIFIED.
A ROOF FALL occurred at Break No. 14 on #2 belt. The fall measured approximately 20' wide, X 70' long and 6' high.
Coal was smoldering and was spotted at approximately 2:00 AM. It was reported to MSHA at approximately 2:30 AM. Steps were taken to extinguish the smoldering coal. The smoldering coal was extinguished at approximately 8:30 AM.
Alleged face ignition on the #1 right working section in the #1 entry.
The 1 North Mains elevator knocked power on a drive fault entrapping one miner for longer than 30 minutes. No damage to the hoisting facility was found during an examination. The entrapped miner sustained no injuries and was entrapped for approximately 55 minutes.
Coal was smoldering and was spotted at approximately 8:15 AM. It was called into MSHA at 8:40 AM. Steps were taken to extinguish the smoldering coal. The smoldering coal was extinguished at approximately 9:55 AM.

Foreman and Hydrator operator were in lunchroom when they herd a ""pop"". In seconds they were outside to investigate. The area near the Hydrate MCC and bag storage area were on fire, and the fire was growing. A brief attempt to control with extinguishers was abandoned and 911 was activated. No injury.

A roof fall occurred in the D Bleeder section at #88 crosscut 3 left entry at 2 room. The fall was 10 feet above the anchorage point where it appeared that a slip occurred with laminated shale.
Governor switch tripped 20 feet off the bottom. Entrapped 21 people until 9:30 am. Schindler elevator services was unable to reproduce the fault. Elevator returned to service at 12:25 pm, when K order was lifted.
A fall of the roof above the anchorage was discovered along the mainline track at 15-08. The fall was 92 feet long X 16 feet wide X 12 feet high. No injuries, travel was affected.
While pillaring the PCC Mains mining in the #4 entry second lift on the left side a large rock fell out and pinned the head. While trying to work the miner pivoting back and forth the breaker knocked on the side of the miner. A retrieval plan was implemented to use a retriever to pull the miner out from under the rock.
The main electrical power that supplies the mine had been off. When the electrical power was restored, the elevator would not operate. No one was underground at this time. MSHA District 6 was notified of this condition.
Contractor Vehicle eletrical fire
Operator was running a Volvo 330 Backhoe when MHST inspector arrived. he had operator stop his equipment to inspect. After inspection operator started hoe back up. Foreman was driving into the pit when he saw engine on fire. Tried to put out with 3 fire ext. called 911 & 800 for MSHA. Fire dept put out fire. Engine was totalled.
On 11-20-13 operating at shaft sinking operations at mine. Mine experienced a power outage coming from the sub station in Yerington, NV. Troubleshooting problem & determining power may be our for awhile we notified MSHA @1303 (EE & another EE were currently conducting a inspection at the time) and removed the miners via Grove RT890 crane and a manbasket.
A roof fall occurred in the D Bleeder section in the 1 left entry intersection just outby the #8 bleeder shaft. The fall was 1 to 12 feet above the anchorage point where it appeared that a slip occurred with laminated shale.
Operator was working to unbridge #26/27 feeders on R-5 Reclaim. He was working the south side of the feeders, pushing toward #24/25 feeders. He was backing up for another push, when he misjudged his location and got too close to the bridged #26/27 feeders. As a result the dozer broke through and fell into the #26/27 feeders.
Roof fall at crosscut #1, 7th Right Sub-Mains, entry #5. Approximate dimensions: 18 feet wide, 19 feet long and 12 feet high.
Roof Fall at crosscut #18, Main West section, #3 entry. Approximate dimensions: 20 feet wide, 30 feet long, and 10 feet high.
The main mine transformer lost power due to two blown fuses on the buried feeder cable to the transformer. These fuses are located on Morton's pole directly below the overhead disconnect switch. This feeder cable supplies 34.5kv voltage tracked through the porcelain insulator of the fuse holder to the steel mount bracket.
Cementation's #4 Shaft Hoist experienced a power interruption determined to be a converter board. There were seven (7) men on the shaft bottom at the time of the power interruption. They were evacuated from the shaft bottom by means of the Egress Hoist. Approximately two (2) hours later the Hoist was repaired, ran, safety checked, and returned to service.
The section foremen on unit #1 called out at 7:28 a.m stating he had a fall inby his tailpiece located in the 4th south west submain (spad 21+20) xc # 28 entry # 4. No one was injured, he did have a change in ventilation. Called and reported to MSHA 7:30 a.m. Contacted state agency at 7:36 a.m. Approx size 19' in length x 19' width x 6' in height or greater.
11 people entrapped on Metz elevator until 7:14 pm, elevator returned to service at 8:00 pm. It is believed vibration in the counterweight guide rails caused the buffer switch to trip and then the over travel switch to trip. Over traveled 1 foot at top of shaft.
A roof fall was found in the primary escapeway of our S7-1st Right Panel in the Primary escapeway. The fall was found at 11:55pm and phoned into the MSHA Hotline at 12:00am. The fall did not cause any injuries to miners, impede passage of miners and did not affect ventilation.
At approx. 5:35 pm, a roof fall above the anchorage was discovered in the intake entry of the 6Left off 4North section. The roof fall was in the intersection at 14+00 and continued outby for approx. 40 ft. The roof fall was approx. 40 ft. long x 17 ft. wide and 6.5 ft.high. There were no injuries or property damage. The fall was dangered off and the escapeway re-routed.
A roof fall was discovered at approximately 2:00 pm in the 2 West Mains primary escapeway. The fall occurred at break 15 in the #6 entry of the 2 West Mains. The fall was approximately 35' long, 18' wide and 6' high.
Associate was cleaning a face when some material fell onto his loader.
The small 2 man elevator was traveling from the bottom to the surface with a man onboard when it got stuck approximately 400 feet from the surface. The power cable at the elevator was pulled loose when it got hung on a bracket to the cross beam.
Elevator stuck 7 feet from top of shaft. Cable lubricant squeezed out and tripped the governor wheel, tripping the brakes. One employee was entrapped until 6:37 pm, elevator returned to service by 7:57 pm.
An unintentional roof fall has occurred in Neutral Entry on #3 Belt spad 3139 - 3159 inby. The roof fall was approximately 40' x 6ft. There were no injuries reported and no equipment involved.
A roof fall was discovered at Wana Bottom location 3 blocks north of the Wana Elevator on the Main Line Haulage. No persons injured and ventilation not disrupted. Area was dangered off and brows supported. Fall will be bolted and haulage restored.
A roof fall occurred in the return escapeway of 2RWest Lease continuous miner section near the overcast near x-cut #2. The fall was 10'x 15'x7' high. 3-7' roof bolts fell when the top fell. MSHA notified at approx. 8:30 a.m. MSHA insp. were on-site when the fall was discovered and were aware of the fall prior to hotline notification.
Unplanned coal dust ignition in the #1 Coal Ball Mill system occurred. No injuries occurred. Property damage occurred to the dust work.
We had a roof fall at x-cut 101 in the belt entry #1. It was 27 feet long, and about 10 foot high. This is on the longwall unit, Headgate #2, 004 MMU.
While it was being used to lower a trailer load of cinder blocks into the mine, the hoist went into an overspeed condition that resulted in the cage being damaged. The cage was manually returned to the surface where it was noted that the cage rollers were out of the guides. The cage is still under repair at this time.
A rockburst hit the 08 stope at 8:32 am with no damage occurring. a small series of rockbursts continued for the next 45 minutes. around 9:20 am the decision was made to withdraw the miners from the stope for precautionary reasons. The stope settled down and afternoon shift went back into the stope to work.
A roof fall was discovered on the longwall Panel 2 belt line in 2 Headgate at 7:50 AM. The fall was located between 89 and 90 crosscuts. The fall measured approximately 50 feet long and 7 feet in height. The fall area had initially been bolted with 6' torque tension bolts. The limestone typically present in the anchorage zone of a 6' bolt had thinned out in this area.
An inundation of gas occurred on the #2 Section while retreat mining. There were no injuries to miners or damage to equipment during this event.
Their was an unplanned roof fall at the 1 st North Main # 1 entry
While conducting his weekly examination the fire boss identified a roof fall in the #9 entry near spad #6887. The fall was approx. 20'W X 160'L X 4'-5'H the area was supported with 4' fully grouted resin bolts. The area will not be cleaned up and the area has been dangered off and timbers or cribs set.
Yazzie pit has been inactive for a couple of years. Some coal seams are expose to ambient temperature and environment. With recent monsoon rain a spontaneous combustion occurred on 8 seam coal.
An unintentional roof fall--Off the Section-Spad #52-Old #1 Belt at Brk 13 in #5 entry-Intersection in OLD travelway-fell 18' long X 12' wide.
An unintentional roof fall has occurred on the #3 Section #3 Entry on Old 6A track 30ft outby Spad 13639. The fall was approximately 50ft x 30ft x 5ft. No injuries reported and no equipment was involved.
At approximately 2:30pm the Schindler elevator became inoperable. The Schindler Company was called on site, electrical repairs were made, and the elevator was back in operation.
415p supervisor notified of strange sound at #3hoist. After investigating the noise it was determined to be coming from the grid coupler between gear box and drum. Evac started @430p out #1 shaft so the #3 coupler & grid could be inspected. MSHA Notified at 445p, all out by 545p. Grid was intact but contacting coupler case. Grid adjusted & hoist returned to operation @645p.
A rock fall occurred on 11/7/13 in #3 entry approx. 70ft long x19ft wide x 9ft high in the 1st East Submain at 4+30. The rock fall has been cribbed around.
The 95 ZIV loader was going up a hill with a load of sand. The operator heard a loud noise from beneath the motor. He started backing down the hill and the loader burst into flames.
An unintentional roof fall occurred at the 5WLW tailgate, entry #3, crosscut 42. The fall measured approximately 18 feet in length, 18 foot in width, and 20-25 feet high. The fall will be mined through, during longwall mining, and the fall will become part of the gob line.
A fire ignited 36"" below the production shaft collar. Two employees were cutting a 2' X 2' hole in the grating around the shaft collar to insert a tugger rope to lower material. Hot slag dropped down the shaft and ignited a bleeder 36' below the collar.
Electrical short in a 4 gang outlet box. Maintenance shop is a total loss along with 4 pieces of equipment that were inside it.
A roof fall occurred just inby survey station 6486 in crosscut from 3-4 entry and extended through crosscut of 7-8 entry of the 3 Right section. The fall is approximately 20 feet wide x 250 long x 15 feet high.
Their was an unplanned roof fall at the 2nd West Main at cross cut 4.
A fatal accident occurred on the longwall face around #144 shield. A ladder line hose got pinched under the toe of the #144 shield. EE did not see hose under shield and initiated the electronic push of the pan-line. The hose that was pinched under the toe of the shield, stretched by movement of pan-line, broke apart and high pressure emulsion fluid struck victim as he traveled by.
Employee was working near an energized Soft Start Component. Started convulsing or having a seizure. Possible electrocution. Still unknown.
Employee went back to check on a post blast and became overcome by Carbon Monoxide from powder smoke. Carbon Monoxide

Employee was operating a haul truck transporting material from the wash bins to a stockpile located Northwest of two settling ponds. On the return trip to the wash bins, the haul truck traveled through a berm on the left side of the haul road, went over an embankment and overturned on its left side in the settling pond.
Employee was shoveling coal into the Longwall pan line. A piece of coal fell from the face striking the employee. Employee was standing under shield #123 on the Longwall face at the time of the accident.
The plant technician's right little finger tip was amputated when she was inspecting the elevator roof access door switch and her finger was pinched between the door and the frame.
Employee cut the tip of his finger off when trying to load a pipe into the back of a pick up truck.
Employee was installing roof bolts when his finger was caught between the basket and the ratchet.
Dozer Operator was stuck. Cat 426 was operator went assist in removing the extraction cable from Dozer 425. 530 scraper was connected Dozer 425. Operator of 425 went to help disconnect cable. 426 operator got between the cable and the dozer blade. Scraper operator 530 started to drive away. Miscommunication between operator and Supervisor.
Employee was with 3 production assistants removing a support beam from the kiln firing hood. They were using a metal cable, pulley block and forklift to pull the beam. Employee was holding the cable to guide it through the pulley block. The cable moved and his hand was pulled into the pulley block causing serious crush injury to right hand. Job was stopped and EMS was contacted.
EE was changing a bearing on a tail roller when his right middle finger got caught between the bearing housing and tailpiece frame.
EE was attempting to remove slack out of a hoist chain on a drill, he pinched his right middle finger, amputating the end of his finger. He was taken to the ER and they sewed his tip back on.
The employee got his left index finger caught between the bag conveyor platform and the bag conveyor head pulley while reaching for a empty bag that came across the conveyor.
Employee was changing out the K410 DDC burner gasket. While installing the gasket between the 8"" flanges the pry bar slipped and the flange closed together pinching the end of his index finger on his right hand. The end of the finger was cut off and surgery performed later that afternoon. Employee returned to the doctor on Thursday, 11/7 for evaluation. Could be off 2-4 weeks.
Employee was spot bolting 10 foot cable bolts. One piece of steel was in the hole when he was pushing a piece of steel up to connect it together he got his middle finger between the steel and the top cutting the end of his finger off.
Getting back on loader going back to lunch. Twisted left knee.
Employee was embarking on haul truck when his right foot slipped on the step causing his left knee to strike the truck.
Employee was helping position the crude oil rack on the next rail car to be loaded. When the rack was close to being in position, employee stepped on a yellow concrete block to signal the driver to stop. Employee slipped off the yellow block and fractured his left ankle. Employee was driven to the hospital and treated.
EE was attempting to clear the 7x20 discharge chute when he was struck by several rocks ranging from 4"" to 8"" in diameter. He was struck on left shoulder & elbow.
Was using a Bobcat, cleaning the drainage area near the elbow dump area. When mud fall from his load hit the ground, some hot dust/steam came-up to his face and ears.
Employee was attempting to pull a piece of rock from the mine roof. When the rock fell it landed on the slate bar that had slipped from the employees hand and the slate bar bounced back up striking the employee in the upper right thigh. This caused a laceration that required sutures.
Injured employee was prying on a take-up cart to break it loose. After he broke it loose he stated his back was sore. He finished the shift with pain in his back. Employee did not begin to miss work until 12/10/2013.
The injured was working on 23 right No. 3 hdg. at the end of the track at time of the accident. The injured was helping adjust the width of the forks on the mini track, When one of the forks caught his left ring finger and mashed it.
The employee slipped on ice and fell twisting his leg.
EE was standing on trailer 5-541 reached up to grab onto the side of conveyor 14-1000, lost his grip, slipped off the trailer and twisted his left ankle.
Employee states that he injured his back while loading materials.
Employee was a passenger in a mantrip when it hit a hole causing the employee to strain his back.
Employee was taking down supply doors at Brk 87 on the long wall. Employee laid the first door against the rib. As they were dismembering the second door, the door against the rib fell, striking employee on top of his right foot.
While lifting a guard off a conveyor belt, employee felt a pull in his groin area. The following day, he reported to his supervisor having pain in his groin area. It was diagnosed as a hernia. (Employee was on C Shift which started 11 p.m. evening before injury.)
Employee was changing batteries on a ram car when the jumper cable caught his arm resulting in a strain.
EMPLOYEE WAS FABRICATION STONE. WHILE REMOVING STONE FROM THE AREA, HE SLIPPED AND FELL FACE FIRST ONTO THE ROCK HE WAS HOLDING AND HIT HIS NOSE AND MOUTH.
He was supervising the re-railing of an off-track dolly. The inby motor struck the inby dolly pushing the off track dolly over, striking his left foot causing a fracture.
Employee walked around the Continuous Miner to operator side of miner when he heard the roof or rib pop, the employee turned to move and tripped and fell on right knee causing injury to knee. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOSING TIME UNTIL 12/13/13 DUE TO SURGERY ON KNEE.
Employee was tramming the fully loaded #9 Auxier Welding off standard shuttle car to the feeder when he went thru a dip in the haul road. He came out of his seat and struck his head on the canopy roof causing pain in his mid to lower back.
While splitting a stone on the guillotine, a piece fell off the rollers and onto his feet.
Employee stated while hooking up a pull duster behind a battery powered, rubber tired ride the duster rolled forward and caught the employee's hand between the duster and the ride.
Maxium Engineering Contractor's spadman was entering the portal and struck his forehead on the underside of the portal canopy. He stated that he was on his knees when they entered the portal canopy on the two man personnel carrier. The impact caused a laceration above his right eye requiring stitches.
Employee was lifting and stacking 30 pound boxes onto pallet on to the floor.
Injured was strapping down supplies on a supply trailer and slipped and fell injuring his shoulder and wrist.
Employee states he was maintaining electric and kerosene heaters at the site the longwall emulsion building. While approaching the stairs to enter the site employee states he slipped on ice on the stairs and fell into the hand rail. The handrail struck the inside of his arm and his chest, ribs on the right side. Employee did not start missing work until 2/21/2014.
Employee stepped on a lifting bracket while installing a cover plate to get better leverage. He slipped off the bracket and sprained his ankle when his left foot hit the floor. Cause of injury was lack of attention. Miner proficiency may have been a factor. PPE, company rules or mining equipment was not a factor.
Was drilling hole in preparation to install rib bolt. Drop the boom on the bolt he had prepared for installation and caught his left foot between the roof bolt and the mine floor, causing a contusion to the left foot.
Employee was marking hydraulic leaks on U/G grader. climbed on top and caught left hand between the tandem tire and the fender bracket, causing a laceration & fracture to the left small finger.
Employee was striking a piece of metal that had become stuck between the roller on the 10A belt conveyor and the chute with a sledgehammer to try to dislodge it. The sledgehammer head bounced off the metal and the sledgehammer head and shaft struck the employee in the mouth, chin and cheek.
Employee stated that he felt pain in lower back while moving 3 inch air pump about 6 feet to a location where it could be torn down.
Employee removed a cover to unchoke a pipe located below Kiln #1 Primary Dust Cyclone. Upon removing a cover plate on the pipe, a large amount of dust was expelled into the atmosphere. Dust got behind the employee's safety glasses and entered both his eyes.
The injured was constructing a stopping measuring 8 feet high X 18 feet in width. The stopping was approximately 3/4 built when he turned away from the stopping to pick up another cinder block. While turned away several blocks from the partially built stopping fell striking the employee on the right leg just above the ankle.
EE was helping carry a piece of medal when he said his left foot slid sideways causing him to twist his left foot. Dr. report said he had a broken bone going to his little toe.
Employee was standing at left side bolter station of 22D miner. Miner advancing to make another cut. A foot jack pin broke, got caught in the miner tracks and was thrown out, striking the employee in the lateral side of his right foot. Caused contusion.
Holding roller chain with hammer while another employee was attempting to knock out a connecting link with a hammer and a sliver of metal struck employee under his safety glasses
Employee stated he lifted drill feed jack, and he twisted his body and trunk and felt a tingling sensation in his back. He continued to work until 12/17/2013 when he started to miss work due to back injury.
Employee was descending a ladder when his foot slipped and his right knee hit the ladder. EP Minerals was not notified at the time of the accident and the injury was not confirmed work related until 12-5-2013.
While starting to remove tubing from a loader bucket the employee stated he did not realize his right foot was under the bucket as it was being lowered to the ground.
AN EMPLOYEE REAGRAVATED HIS BACK WHEN PULLING A WELDING CART.
EE was lifting the end of a piece of sheet metal to slide it out of the way. When he picked up on the metal he felt a sharp pain in his lower back.
A pit millwright was struck on the head/neck by the handle of a sledge hammer when it was improperly being used as a spacer between a hydraulic ram and an idler bearing block to adjust the tension of a shovel track.
Employee was sitting on a pallet of blocks with his back turned to a co-worker who was cutting plastic bands off of the block pallet, he turned his head to see if his co-worker was done cutting the bands and the cut band flew up and hit him in his right eye causing a cut in his cornea.
EE states he was hanging Miner cable in #5 entry on # 4 unit causing pain to middle of back.EE failed to get help while lifting cable.
Employee was B bonding the back side of a stopping at Brk. #33 of #1 entry crosscut of 6 belt when a piece of draw rock measuring 46' x 46' x 2"" thick fell out between bolts striking him on the back side of both legs.
Employee had finished changing a dust suction on the roof bolter. Asked the drill to be started and the head started to raise slowly pinching his fingers between the long and short arm. The up lever was partially engaged to cause the head to raise. 2 fractured fingers
EE worked a full day on job, did not say anything to crew members about getting hurt. When they arrived back to the shop after work he left for the day, never saying anything about hurting. About 9:00 p.m. that evening he called owner to say that he was going to the hospital because he said he had a bone sticking out of his hip. When asked what he did he said he did not know.
Employee was traveling the longwall face and stepped over a ram bar. Felt pain in ankle. Sprained ankle
Employee stated that he strained his right shoulder while attempting to mount a grader.
Injured employee was attempting to drag a ventilation tube to the face of the #1 entry on the 8 Right section when he stepped on a lump of coal and twisted his left knee resulting in a sprain.
EE went to get wood block from the bed of the pickup truck. He reached over the tailgate to pick up the block and pulled something in his right shoulder.
Employee was installing steel plates to cover damaged areas of the silo wall. Holes were drilled into silo wall for anchor bolts to hold steel plates into position. While holding a steel punch with his left hand, he swung a 3 lb. hammer with his right and missed the punch, striking and fracturing his left thumb.
While knocking pins out of the Right Angle Conveyor of the Pasco Highwall Miner with a slate bar the the pins came out causing the slate bar to strike the employee in the hand and face.
EE WAS PUTTING AIR IN THE LEFT FRONT TIRE OF THE TRACKMOBLE. UPON COMPLETION EE STARTED TO MOVE AWAY FROM THE TIRE. THE SPLIT RIM FAILED AND SHOT OFF THE UNIT. IT BLEW DIRT AND RUST PARTICLES INTO HIS ARMS, CHEST, STOMACH, AND LEGS. THE RIM HIT HIS LEFT LEG AND HIS KNEE SWELLED UP. RIM WAS RUSTED BUT DID NOT SHOW VISABLE SIGNS OF STRUCTURAL ISSUES.
Employee placed his left hand in door jam of loader, door wasn't securely latched which allowed the door to slam shut breaking his finger.
Employee was exiting a shuttle car when he twisted his right knee. He continued to work until 11/25/2013. At that time he was taken off by a doctor and began missing work.
Employee stepped on to the man-trip when his knee locked up, he did not slip or trip and there is not a clear cut event that lead to his injury, we believe that his years of underground work had most likely weakened his knee.

Employee was shoveling coal spillage from around a conveyor belt and his wrist began to hurt.
While getting off a 20 ton motor he twisted his left ankle and strained it.
Employee was assisting to ballast the track rail and crossed the parked equipment between the ballast cars, getting his feet tangled in the hydraulic hoses which operate the ballast cars. Employee then slipped/tripped and started to fall, reached out to catch himself and over exerted his hand and wrist.
Employee was working from his knees. As the employee stood up, he felt a sharp pain in his right knee.
Employee was assembling highwall miner-using sledgehammer & slate bar. Was prying with slate bar to align bolt holes on structure of 2nd deck, bar slipped & he fell on deck onto his behind.
Miner went against training and stuck head in pinch point between counterweight and frame of excavator while installing counterweight.
Mechanic received second degree burns to his left palm when he touched an air arc rod that he was utilizing to remove a bushing from a Komatsu PC 3000
The IE was loading roof bolts on the offside of the bolter. When the operator started to advance to the next row, he heard the IE scream from his foot being ran over. The IE was taken to the hospital and treated for a contusion on the top of his foot. The IE continued to work, and then on 4/8/14, IE was seen by Dr. and failed to report to work on his scheduled afternoon shift.
This was an accident that occurred with prior employee under prior management. I have spoken to the former employee to gather this information. On November 15, 2013 the employee was removing a cover from a conveyor and broke his pinky finger on his left hand. We no longer own the conveyor that was involved in this incident.
Employee was changing oil filters on his loader and slipped and fell injuring his arm. He fell 38 inches to the ground.
He was walking on the 8 south section when he tripped and fell landing on his right knee. He started missing due to the injury on 11/18/2013
EE said he thought that maybe his boot was wet and that caused his left foot to slip while mounting his Euclid Water Truck (26.3604), causing pain to his upper left leg. Injury was not reported until 6:45 AM on November 18, 2013 stating that he needed to be off for a few days until he got better. Co. advised EE to have medical treatment.
The foreman turned and exited the pick-up truck and upon placing his feet upon the ground, he experienced pain in his lower back and the front of his thigh that radiated downward past his knee.
Employee was walking to his shuttle car, while walking bent over (48"" coal), the employee struck his head off of the rib and caused a stinger, and pinched nerve in his neck.
Employee stated that he was riding on a golf cart with his right hand resting on the golf cart fender. When the golf cart passed through a back-up curtain his right hand was caught between the golf cart and a ram car that was parked there. This smashed his right hand causing a laceration on his small finger, that required sutures.
Employee states that while operating a 785b, he was backing under the loader when the 992d bucket struck his cab protector. He states that this occurred several more times during this shift. Employee continued to work until 12/04/2013 when he began to miss. He is complaining of low back pain.
Employee was in the #3 portal rock dust borehole near the track mounted rock duster. The injured trip over the track rail and tried to break his fall. Injured his shoulder.
Employee bent over to pick up a switch and could not stand back up.
Employee was walking in Entry 6 of the west submain with his apparatus on (Mine Rescue Training). He stepped on a small piece of coal covered with rock dust in the walkway, causing him to roll his ankle.
Injured employee strained his left shoulder and back while loading a tool box on the 8 Right section. Employee began to miss work on 12/10/2013.
As the employee was exiting a pickup truck, he slipped on the ice and fell resulting in a sprained right ankle and a contusion to his right knee and back.
Employee fell backwards from an elevated platform and hit his head on the ground causing a concussion and laceration to his head.
* Late report by EE. EE initially stated that did not know time & date of injury it was determined non-work related. However after investigation & request of medical from the treating Dr which was received on 12/12/13 the EE reported to the Dr that on 11\15\2013 at 5:00 p.m. he pulled on a metal step and felt discomfort in his groin. He was diagnosed with an inguinal hernia.
Employee was changing out belt, he was climbing up on structure when he twisted his right knee. Employee started missing 06-23-14 due to knee surgery.
Roof Bolter on Unit#5 was trammed up on the right foot of EE by another ee who was at the side tram controls. EE was leaning over the mid section of bolter on the right side of bolter. He was cleaning off space for some bolts and did not hear bolter start up due noise from forklift.
While walking, employee slipped in a tire rut and twisted his right knee. Continued to work all scheduled shifts until being taken off of work 2/25/2014
Employee was hanging curtain while a co-worker was striking pin steel against the bolter. Co-workers steel flew out of his hand striking employee in the face.
Injured employee was walking through a set of scoop doors on the 8 Right section when he struck his head on a cable bolt and felt pain in his neck.
Employee slipped on wet surface and fell to ground. Employee complaining of right shoulder, lower back and right knee pain. Employee placed on restricted duty on 11/26/2013.
Employee was shoveling the slope belt when he felt pain in his back.
The employee had left the face area and was walking to get on the mantrip. The employee slipped and fell hitting on both knees. There were no witnesses to the accident and the employee failed to report his injury until the following day at 6:30 a.m. by phone. The miner states he can't put his weight on his left knee.
Employee was bolting a track switch when hit by a mantrip. At #10 Belt Drive switch. Transferred to Hospital. Surgery to remove spleen, fractured pelvis no surgery. On ventilator 4 days, sent to Health South therapy, home 12/13/2013. No return to work date.
Working on grease bucket on Cat 5130 B Hydraulic Excavator when he slipped on tracks & caught himself with his left arm. Causing Injury.
Employee was climbing the stairs to the cab of a haul truck when he felt a pop in his left knee.
Employee was traveling through a mandoor and stepped on a piece of a broken hollow block, causing him to twist his ankle. X-rays were negative however the employee was held out of service by the medical provider.
Employee was exiting the skid loader when he fell, fracturing his wrist. After the accident, the foreman drove the injured employee home and he was then laid off. Allegheny Mineral did not find out about this incident until 1/24/14. Once Allegheny Mineral confirmed that this injury occurred and was not reported, the foreman and injured employee were both terminated on 1/27/14.
EE was getting ready to go down stairs, he tripped on the top step falling down approximately 7 steps to the landing. He fell with his right leg bending backwards underneath him. The Dr put him on crutches and sit down work only until he has an MRI on Nov 14th.
Employee sustained an injury while working when he hit his right elbow on the steel structure beam that lead to Cellulitis in his right elbow.
Employee stated that he crossed the overland belt at the crossover and as he dismounted the last step his foot slipped and he felt something pull in the rear of his right ankle. This resulted in ruptured right Achilles tendon. Employee continued to work until 12/2/2013.
Employee was helping set the continuous miner up for the next cut, he bent down to pull the miner cable out of the roadway he then felt a sharp pain in his genital area. He finished his shift and sought medical treatment the next day and was treated for a strain.
Employee was installing a 8"" roof bolt and he felt a pain in his groin area. The employee worked for two day's before going to the Doctor. The employee started missing on 11/15/2013.
While refueling a road grader the operator was exiting the machine coming down the ladder and slipped off the step and fell onto his right wrist.
Employee was hauling supplies to the section. Employee crossed the track with scoop causing his head to hit canopy jamming his neck.
End of shift, washing out skid-steer bucket and pins; when employee pulled out pin the bucket fell on his left foot. Fracture left big toe bone. Employee remains off work
Employee stated on 11-11-13 he injured his back lifting cinder block. He finished his shift and went to Logan ER and received a full release. He RTW on 11-12-13. On 11-16-13 he went to CAMC and was dx. with Sciatica and He requested time off work. On 12-11-13, we were advised by OIEI that this incident was reportable.
EE drove 970 loader all day and at the end of his shift said that his back was jarred from operating the loader all day.
Employee was working on grader blade trying to put it back on. Blade kicked out from jack and hit his foot.
Employee was trying to remove a drill steel from a 988H loader bucker which was wedged in the bucket. He hit it with a hammer and it sprung out hitting him in the ankle. The doctor did an x-ray 11-12-2013 and gave him time off until follow up appt. on 11/18/13. At that time, the Dr returned ee to light duty with anther follow up appt in 10 days.
Employee was changing a tire on supply hauler, the tire rolled trapping arm between tire and hauler fender. The employee continued to have complications with arm and was placed off work March 12, 2014.
Ee used his own body to try to push bent handrails back into place. He braced his back against a rail and pushed hard with his feet against the bent handrails. After working remainder of shift, went home, slept, and woke with pain the next day. Seen at clinic, diagnosed with upper back strain, released to restricte duty.
While clearing himself from pathway of buggy cable, operator turned on buggy and the cable caught employees' leg and flipped him.
Details surrounding the incident are uncertain. Contract driver claims to have fallen approx 15' off metal stairs at loading area. Driver left the premises and delivered at destination approx 250 miles from plant. After unloading, driver reported to the trucking company next day. Texas Lime was made aware that this incident led to MSHA reportable criteria on 4/1/14. DOB Unknown
Injured employee was attempting to hang the continuous miner cable and water line when he felt a pain in his left lower abdominal area. Employee was diagnosed with a strained left groin.
Employee was in the process of removing hoods on haul truck and pinched his #1 & #2 fingers on Left hand. He received sutures/restrictions were issued.
Walking to the manbus to get a drink of water. He fell striking the ground.
Employee was pulling on a jammed gate. When the gate gave way, the gate pinned his right thumb between a large bolt.
Ee backed into be loaded and shovel door came in contact with headache rack, jarring the employee in the cab. Ee complained of numbness in legs and tightness in upper back. Initial evaluation resulted in referral to specialist, specialist released employee to restricted duty which could not be accommodated.
While employee was hauling shield he hit a pothole on the road and felt a pinch in his back. He continued to operate for approximately 2 more hours and his lower back began to hurt worse, so he contacted his supervisor at approximately 6:15 pm
The injury occurred when the contractor was struck on the left hand by a crib block that had fallen from an adjacent set of cribs that were being erected. The contractor received a fracture to the left hand. Cause of the injury was working below areas where objects could fall from above.
Injured was cutting a bad section of rockdust hose out with a razor knife, the blade broke. This cause the knife to slip off the hose cutting left wrist. Cause not using the right tool for the job. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs could be a factor, miner proficiency was a factor.
EE was tramming 4-wheeler to section, hit a bump and hit head on mine roof.
Lifting bucket of material from #1 elevator Pit.
Bolter operator was having problems with the dust collection system on the bolter. As the operator cleaned the hose under the pot. The utility man on the section noticed the problem and came to help. He stuck his hand in the pre-dump on the bolter as the operator sat his foot jack. This activates the pre-dump. This fractured his right arm.
While in the process of shoveling belt employee strained his lower back.
Was driving loader and the steering wheel jarred hard and jammed his left thumb.
While pulling slack to hang cable the employee felt pain in his lower abdomen. He continued to work until 11/11/13 when he was placed off work with an abdomen strain or sprain. The employee currently remains off.
Employee was helping electrician remove rub rail on miner when a piece of rib fell and struck the right side of his middle to lower back. Employee continued to work until 11-18-2013 where he was taken off work due to his illness.
EE was cutting, welding and grinding on a scoop frame and some time during the process a sliver of metal became embedded in his left eye.
This office was notified on 12/4 of this injury. EE states on 11/8/13 he was carrying AX rails, was bent over to get them through an area 42"" high and struck his head on roof bolt strap feeling pain in his low back/legs. EE sought med treatment on 12/2, was taken off work for 1 day on 12/3 and returned to modified duty 12/4/13. This is an aggravation of an existing injury.
Employee had been in the restricted area of 50% sodium hydroxide solution to turn the caustic system on. Employee was wearing all his PPE while in restricted area. He completed his task and left the restricted area to remove PPE. A hose on the system burst as employee was walking away after removing PPE. Employee received chemical burns to back of neck and back of legs.
Miner operator was pinned between standoff cable handler of a continuous miner and coal rib in #6 Entry of Productivity Mains. Cause miner operator not paying attention. PPE was not a factor, compliance rules/regs was a factor, mining equipment/sytem was a factor, miner proficiency was a factor.
Employee was lifting a steel liner plate and strained lower back.
Filling machine with oil while standing when he lost his footing and slipped and caught himself causing injury to his back.
Employee stated that he stepped into a hole in mine floor and his knee hurt.

Employee was rolling up a 1 1/2 in. water hose from filling up the water cart when it flipped up striking him on the right side of face and eye. This injury resulted in lacerated sclera.
Was heating and bending roof bolts into hooks to drag track rails with. Felt a pain in his right hip area. Started missing work on 11/11/2013.
Employees were using an Elliot Aerial Platform to perform scaling work when the man basket they were in fell to the ground.
Employee was bending over to attach fish plates to track rail when he experienced pain in right side of neck and shoulder. Employee was treated at Raleigh General Hospital. Employee continued working until 11/20/2013, at that time the physician instructed him to be off work until further notice.
Employee had clocked out at the end of his shift and was walking out leaving the plant. He stepped down off the side walk and on to the asphalt drive. He lost his balance and fell to the right knee. He went home, went to bed and when he awoke he went to his doctor. X-rays showed a fracture to the knee cap.
Employee was fueling his equipment from a tank at the mining site. Employee was walking down inside of fuel containment berm when he slipped on the exposed plastic that had frost on it from the cold weather. Employee fell onto back compressing/fracturing his vertebrate.
Employee was roof bolting. A drill became stuck in the roof. He went to check his suction and the steel fell out of the roof striking his hand.
While getting into cab of semi truck, driver ""smacked"" top of head on door frame. diagnosis - laceration and concussion without loss of consciousness.
Coming down ladder from scale house and knee buckled and fell backwards.
Employee was climbing an extension ladder to tie off the top when his foot slipped and he fell about 5 feet, breaking three ribs and collapsing a lung.
Employee was in the process of removing a frozen bottom belt roller. He was using a screw driver and a hammer to break the roller free to remove it. As he lifted the roller he strained himself. The incident was under investigation by Workman's Comp until 12/19/13 when it was determined to be a work related incident.
Injured was moving a golf cart by parked supply cars. The supply cars were moved before he was in the clear. There was block on a supply car that caught the seat on the golf cart pushing him forward which caught his leg under it causing a fracture. PPE was not a factor, compliance/rules was not a factor, mining equipment/sytems was not a factor, miner communication was a factor.
Employee was moving a roller when his foot slipped causing him to jam his knee. The employee continued to work until he was taken off for a non-work related injury to his foot. While off work because of his foot he had surgery to repair a torn meniscus in his knee on February 12, 2014. He is still off recovering from his foot surgery. He is not off work because of his knee injury.
Employee pulled muscle in upper right arm while picking up material to move. Employee did not start missing time for this accident until after being examined by a physician for arm pain and it was determined the pain was the result of November 6, 2013 accident. First day missed January 13, 2015
Employee was helping to install a hi-speed roller with a slate bar. The roller slipped on the slate bar and his right hand became caught between the slate bar and belt structure.
This accident did not happen on the mine site. Contributing Factors were: Rough walking surface (Grating) Worker stumbled on grating while desending railcar. Injuries: 3 Fractures on right foot. Involved railcar NAHX 60563

A coal truck operator was waiting to get loaded by a loader. The loader dumped its first bucket into the truck bed which caused the truck operator to get bounced in the seat. Employee was taken in for physician evaluation.
Employee was unloading trash. He picked up a set of old wooden steps and sat the steps down on a damaged belt cradle and pinched his left index finger between the wood and metal.
EE was retrieving a come along and belt brackets from the tail section of a elevator he reached down to pick them up and felt that he may have pulled muscle in his lower stomach. EE did not report the alleged incident until 11/18/2013 and did not seek medical attention until 09/05/2014, restrictions were prescribed for a possible hernia at which time the incident became reportable.
Pulling on a Bolt on a 993K Loader & felt something pop behind Right knee.
While bolting off the miner, the employee was struck by a section of roof rock or head coal in the head, left shoulder and back resulting in a contusion. The event became reportable when the employee began seeking medical attention on 11-08-2013 and began missing work.
EE stated when he arrived on the section he picked up a 50 lb bulk box of nails and carried them to the section tool shed. He stated he felt pain in his back at that time. He continued to work the rest of his normal shift. The EE called in the next morning and stated he was seeking medical attention. The EE has not returned to work and is currently under doctors care.
Employee was operating shuttle car and as he was turning his coat sleeve caught the rib and it pulled his arm between car and the rib and bruised his arm in which medical personnel later put a cast on it to help protect the muscle and tendons from future damage.
Employee was exiting his truck at the scales to weigh. When he stepped down on the bottom step, it broke, throwing him backwards and he hit his head and shoulder on the platform. He passed out for about 3-4 minutes. Employee was taken to the ER by the supervisor. This resulted in a concussion and a shoulder contusion.
Employee was working off side of stage loader taking down monorail, when a piece of coal rolled from rib striking his foot causing a stress fracture to the foot.
Employee set up a 8 step ladder on unstable ground. The side he was climbing was on concrete at the other side was in loose mucky coal. Employee went to 3rd rung to check footing of ladder and ladder begin to sink. Employee jumped off ladder and broke his right ankle.
Employee states that while making a belt splice he was using channel locks to pull on the belt, he felt pain in his right shoulder. After a couple of days, the pain and discomfort continued and he went to the Dr. and he was taken off work.
Employee stated that he was performing belt maintenance on the belt head and as he stepped and turned he twisted his right knee. Employee continued to work until 2/24/14 when he underwent surgery to repair a torn meniscus.
The employee was foaming the wall on a square set when he lost his balance and fell into the under cast.
Individual twisted his right knee stepping off the reclaim belt. Individual jumped from the belt structure to the floor.
The employee was walking along a string of 5 rail cars when a second employee, operating a pick-up truck in reverse, hit and knocked down the injured employee. The victim received only minor contusions.
Employee was using a 4-1/2 inch electric grinder while fabricating handrails, he set the grinder on the table before the wheel had stopped spinning, and the sanding flap wheel caught his glove and sucked it into the grinder between the wheel and the guard. The incident became reportable on 11/6/13 when the employee did not report for his scheduled shift.
Smashed left index finger while using the impact wrench to connect bolts to a screen that is used to sift rock.
Employee alleges he was traveling outside in a 4-man mantrip when it hit a hole in the travel way jarring him resulting in pain in his lower back. This is being generated pursuant to the Part 50 audit citation #8209370 issued on 8/4/2015. Management disagrees that a violation of the standard cited exists.
Mining in #3 heading of 22D when vent tube fell and struck employee. Injured felt pain in back of neck and shoulders after he got home.
Employee was breaking a track on a D-10 T Dozer, striking the Cat with a hammer with one hand & had his other hand resting on track, Track moved, catching his finger and mashing it. Received 4 stitches and tore fingernail off.
EE was assisting foreman make belt splice, while hammering spindle a piece of metal chip flew off hammer and went into left arm. EE had to have piece of metal surgically removed from left arm.
The employee injured his lower back while carrying big john props.
Employee was cleaning out a plug with a poke bar when he bent his thumb back. He was given first aid at that time but later, on 1/2/2014, stated a desire to go to the doctor. The doctor prescribed Naproxen and gave the employee a splint to wear.
Employee was checking a fuel leak repair on a haul truck when a loose particle blew into his left eye. He was treated by an optometrist who removed the foreign body.
A diesel mechanic was preparing to install a radiator in an OB truck. Was positioned at front of engine and noticed the hood in a questionable position. Pushed on the hood to check if stable. The hood shifted and moved towards mechanic. The mechanic reacted to the movement and turned to get off truck. Mechanic went off the front bumper and contracted hands with the floor.
Employee experienced sharp pain in sacrum area and numbness in toe/foot after pulverizing. This claim is being disputed. It may be related to a previous injury.
EE had parked his vehicle at the warehouse. He reached for his chock block and pulled the calf muscle in his right leg.
While training as a haul truck operator, the technician reached behind the drivers seat for a backpack while kneeling on the passenger seat through the passenger side door when she felt a strain in her back. She was diagnosed with a herniated disc on 11/5.
Injured employee was using a hand held grinder to cut the hinge pins holding a springboard in place on a continuous miner. As the pin was cut the spring was under tension causing it to release and strike his right hand resulting in a contusion.
Employee was installing a core breaker on the rt. continuous miner. He was striking the core breaker with a flexco hammer when a piece of the core breaker broke off hitting him in the left eye.
Employee was walking through cross-cut when he fell down and fractured his left ankle.
Employee was removing a filter from a loader when he strained his right shoulder. Employee did not start missing time from this injury until 11/04/2013.
Employee was checking voltage on each phase of the section power center, for unknown reason the multimeter he was using short circuited producing an electrical flash that resulted in burns to his face and hands.
To begin with this accident was not reported, nor was anyone aware of it occurring until 11/14/13. At which point we put it under investigation to determine its validity. The employee was changing a breaker in a power center and the breaker panel slipped and fell which caused a laceration to his right ring finger.
Emp was starting work when he saw a door open to grease room. Shut door to grease room and walked up steps. Grease on the bottom of his boot caused him to slip on steps twisting his leg. Employee did not miss any work until MRI showed torn tendon. Emp had surgery 12/23/13 and became a lost time reportable.
Employee was allegedly placing a rock into a muck car and the rock broke apart and his hand was struck by a piece of the rock. Doctor issued restrictions

Injured employee and co-worker were installing a return roller at the load out. Injured employee was holding a nylon strap that was attached to one end of the roller. The roller slipped from the co-workers hands - jerking the strap the injured employee was holding. Injured employee continued to work - had surgery to repair a small rotator cuff tear 12/22/2014.
when pulling and lifting a screen, experienced pain in upper arm to shoulder
Employee was spraying off concrete pad under Jaw Crusher. He said he slipped and sprained his ankle. He said he didn't need to go to the doctor, he just went on home. On 12/01/13 he reported to work and worked for several hours, then said his ankle was still bothering him, so he went to doctor the next day.
Employee states that they were on the bull gear reducer platform looking at the bull gear and pallets. As they turned on the platform, their foot slipped off causing them to fall to the floor.
Employee was diagnosed with a hernia when he saw a doctor for a physical checkup. His new-employee physical from 9/18/13 did not diagnose the hernia. After interviewing him, he said that he felt a pull in the groin area while moving a pallet on 11/25/13. He did not report the incident prior to his diagnosis. We assume this is the date of his injury.
Employee slipped and fell hurting his Left shoulder. Employee reported it to supervisor. Employee was taken to Doctor treated and released. On 12/03/2013 employee was not getting better so he was taken to doctor again. Employee was taken off work until seeing the doctor on 12/13/2013.
Miner was tramming a mechanical claw when he hit a rock in the road, which jarred him injuring his neck. He did not request medical treatment at that time. He had his first medical evaluation on 3/19/2014.
Mechanic & welder were in the JLG man basket removing a leveling cylinder pin. Welder was repositioning the basket the machine was shut off & the boom started to settle resulting in the arm sliding forward. As the boom lowered it pinned the mechanic between the upper bucket & the back railing of the man basket resulting in an injury to the mech left ear. Seen at ER received sutures
Employee was changing a hydraulic hose on a front-end loader and injured his left arm.
Employee was performing inspection of yardrunner, tripped on outrigger. Fell on left knee, fracture of knee.

While lifting primary crusher toggle for repair, lifting device broke and employee fell approx. 3-4 feet on top of falling hydraulic toggle.
EE was running the komatsu 450 front end loader feeding the portable plant. EE went over a bump in the road and it shot pain down his back and into his legs. EE then called another ee to let him know what had happened. After talking with ee over the phone another ee had him seen at Urgent Care

Employee reported lower back pain while lifting a piece of grating. He was evaluated by a physician and taken off of work.
Injury occurred on Mon. 11/18/2013. It was not reported until Fri. 11/22/2013. EE explained that he was trying to pull on a stone. His hands slipped and he fell landing on a rock.
Employee was descending down a stairway and missteped, lost his balance which caused him to fall on the landing. He injured his right knee. Employee was taken to the Medical facility where he was diagnosed with a knee strain and taken off work.
EE was rebuilding brakes and moving parts around when he felt a strain in his lower back. EE reported the injury to his supervisor and taken for medical evaluation and placed on modified duty.
Employee stated that while trying to reposition a piece of channel iron he dropped on his foot causing a fracture to 3 bones in his right foot.
On Tuesday 11/19/13 EE reported to his lead, that he was experiencing severe pain in his abdomen area. He said that on Friday he and a co-worker were changing the upper Rotex screens and he felt some pain in his stomach but thought nothing of it. EE had surgery to repair a hernia. He will be off at least ten days and will be able to return to work with restrictions.
Employee was walking on the #1 unit and tripped over the miner cable and fell hitting his head.
EE tripped & fell over a 20""x20"" concrete footing that he did not see. He didn't have any complaints of pain nor told anyone he was hurt. On 11/27/13, he informed management this could be work related & requested to see a doctor. On 1/23/14, an Independent Medical Examination determined the prevailing factor for his back pain was the fall he sustained on 11/14/13.
Injured was attaching hose to the vac truck while standing on the work platform approximately 5 1/2 feet off the ground, when he lost his footing and fell. Employee was wearing fall protection, including harness and lanyard. The equipment prevented the employee from striking the ground, however it caused him to swing back into the truck causing a rib injury.
Myself and co-worker were lifting a piece of sheet steel when the steel shifted away from us resulting in co-worker losing grip and it falling approximately 2 1/2 feet onto my left foot. Resulting damage is a hairline fracture to 1st metatarsal.

Employee was lowering a dozer belly pan, which stopped an inch from the ground. Employee reached under and grabbed the pan to jarred it loose. The pan loosen and fell an inch. Employee pinky and ring fingers were smashed, to which it required stitches to close the wounds.
Employees were using an Elliot Aerial Platform to perform scaling work when the man basket they were in fell to the ground.
EE was positioning plate steel on the layout table and felt pain in his lower back. EE reported the incident as a first aid case on 07NOV13 and declined medical treatment. On 12NOV13 EE was still experiencing pain in his lower back and requested to be medically evaluated. EE was taken to the Dr., diagnosed with a lumbar strain, prescribed PT and returned to work with restrictions.
Employee walked behind the loader without radio communication assuming the loader would take a path that would not put him in danger. Accident resulted in lower extremities suffering soft tissue trauma and sprained ankles.
Mechanic was installing a hydraulic hose on D10R Cat Dozer and over-exerted his left arm, causing him to feel sudden discomfort in his shoulder. On 11/08/2013 the injury became recordable.

Ee was walking through paste when he felt left knee pop, causing the knee to swell. He reported the issue, tried self-care, and sought medical attention on 11/06 at the clinic. Diagnosed with a knee strain and placed on restricted duty.
Employee reported he twisted his knee while performing preshift examination of the Hydrate Plant.
Employee was dumping wet material on the upper west dump when the dump gave way. The end dump traveled backwards over the dump before over turning.
Employee was cleaning up around oil station work area, he stepped on a wooden pallet, twisting and breaking his ankle.
Employee was standing behind foreman as he was cutting the seal/bearing off of shaft on magnetic separator. Something caused pressure behind the end cap on the separator. Before finishing the cutting, the end cap blew off, glanced off of a nearby pipe and struck the injured employee in the forehead resulting in a laceration and fractures to facial area.
Emp.stepped into pump pad flooded with caustic which filled his boots. Went unreported for 2.5 hours. Once notified, supervisor contacted med dept,RN assessed and determined first aid case. Rechecked on 12/1/13, employee off 12/2 & 12/3, rechecked on 12/4 with no signs of infection. Evaluated by Dr. on 12/9/13, & med prescribed for cellulites, resulting in recordable.
While boarding haul truck employee felt pain in right shoulder.
Employee was descending a ladder and twisted ankle. Employee on restricted duty.
An approximately 240 lbs piece of metal was removed from a belt and placed on the floor. The injured attempted to move the piece of metal to a scrap waste receptacle and fell. She attempted to use her outstretched arms to break the fall and suffered left and right broken wrists.
Employee twisted his left knee when he was helping to change a screen. Experienced pain and swelling in knee. Dx: left knee strain.
On 11/27/13, employee was carrying a bucket of parts and felt a sharp pain in the back of his right knee. Employee did not wish to seek medical attention at that time. On 2/24/14, employee was placed on restricted duty.
Injured worker was going down stairs and slip when he went from the last step to the ground. Right quad leg pain. Placed on restriction 12/04/2013.
The injured employee and co-worker started to trouble shoot cone #6 to determine the cause of the cone shutdown. The injured employee started to remove the sensor when it unexpectedly released and released hydraulic oil. The oil caused a laceration to the right hand.
Employees were loading oil barrels into the box on a pickup and the barrel began to tip and the employee tried to catch the barrel and strained his back.
A technician was walking from the weld shop to the tool connex when he stumbled/tripped on the uneven ground catching himself with his right hand/arm. He attempted to catch himself with his fully extended arm, causing a fracture to his shoulder.
9 Dragline crew along with 2 Midstate employees were removing lugs on the drag drum to swap ends with cables. Nuts on the lugs had been removed on cab side cable and Midstate employee was using a bar to pry lug loose from drum. Dragline Operator was in drag pit and had his hand on cable. The lug came loose cable pinched operator's finger between cable and drum.
Mechanic was placing wheel chocks down on fork lift, while walking away, tripped over rope that connected chocks causing mechanic to fall striking right knee on the ground.
EE suffered a laceration/ fracture to his right foot while struck by scaling bar. The injury occurred when EE accidently struck his foot with a scaling bar. EE was taken to E.R. for treatment. X-ray revealed fracture of metatarsal bones in right foot. Laceration required sutures to repair - ACR

Ee was lifting a heavy object and felt a twinge. He had been lifting and moving ramps and on the last ramp, felt the twinge. Seen at the clinic, released to restricted duty.
EE was descending stairs when right foot slipped. EE unsure whether hit foot on framework of stairs or twisted foot but after regaining balance right foot was sore and hurt. EE was sent out for diagnostic treatment and determined EE had a metatarsal fracture to the right foot.

Employee was working throughout the day using an air hammer chipping concrete and pulling chain falls. His left shoulder began to ache but he continued working thinking it would subside, but it did not.
Employee reported lower back pain while moving from a kneeling to a standing position. He was evaluated by a physician and released back to work with restrictions.
EE was leaning over the undercarriage attempting to grab his lifting chain while installing the belly pans. EE was leaning directly over the lifting eye. He grabbed the his chain and was backing out away from the undercarriage when the lifting eye caused a contusion to his left chest area.
Operator had hooked a steam hose up to a washout on a suction line of a pump. The operator was stepping over a pipe to access the washout and tighten the steam hose and stepped into a hole and rolled his ankle spraining it. He was taken to the doctor to be evaluated and was allowed to return to work with restrictions.
Operator was dismounting drill when he slipped and fell hitting his back against the drill.
Equipment operator was conducting his pre operational inspection on his haul truck when he slipped on a patch of ice pulling his groin.

Employee was cleaning up after a maintenance job on the # 6 baghouse. He had bent down to grab the welding and ground leads and a cutting torch. As he proceeded to move forward and began to stand up, he rammed his head into an air pressure receiver pipe that was in clear view.
Injured was cutting bands off of a stack of roof mats. As he cut the band it snapped back cutting his right thumb.
Miner broke right foot while removing bucket from a skid steer.
Employee was closing the service cage door and inadvertently pulled the door into the back of his heel resulting in a contusion. After seeking medical attention the employee was released with restricted duty.
Injured tripped and fell while cleaning up the work site. He tripped and fell. His right hand landed on a piece of glass. The glass pierced his glove and cut his hand.
While getting off the railroad car, his foot slipped on the icy step. As he grabbed the railing to break his fall, he felt pain in his left shoulder and lower back.
Employee was installing a brace for a steel beam when the angle-iron brace flopped over and mashed his right index finger requiring stitches. He was wearing leather gloves at the time. Employee will be on light duty for several days until released by the physician.
While attempting to climb into a front end loader(CAT 966H),an employee slipped, hurting both knees. The employee was climbing in to the front end loader to load the hoppers with barite ore. The employee stated that as he was e entering the cab, his left leg slipped causing his right knee to impact the top of the cab landing.

Employees were repositioning forks on forklift when a stuck fork released and caught the employee's middle finger between the frame and the fork. The employee pulled their finger from the pinch point and fractured their finger tip as well as avulsed a large section of flesh from the finger tip.
Employee was moving the S/C down the belt out of the way of the bolter when he hit a bump in the bottom causing him to strike his head on the canopy.
Employees received a knee sprain while boarding a fuel truck
EE was returning tools and items to a tool box after completing maintenance work on a conveyer. While he was lowering the tool box lid, it slipped from his hands and pinched/lacerated the middle fingers on both hands. EE saw a physician that evening and was placed on restricted duty.
Employee was walking on leach pad. Stepped from a large rock to a smaller one and strained left ankle. Was seen by clinic and placed on restricted duty.
Employee was using a blower to blow dust off his body. As he moved the hose over his chest and up toward his face the blast of air blew his safety glasses off his face and they fell to the ground. Employee believes dust got blown into his right eye at that moment. Around noon, his eye began to hurt and was swollen and bloodshot. Treated for corneal abrasion.
EE was loading boxes of resin to roof bolter when he felt a ""pop"" in shoulder. EE was treated on site but continued to experience pain. EE was seen on 11/19/13 at Occ Med clinic diagnosis was possible rotator cuff tear. MRI was completed on 11/21. No tear was found. EE was prescribed 6 sessions of PT starting 11/20/13. EE continues to work regularly scheduled hours.
A spacer plate that went behind a jaw die fell off the wall of the jaw, landed on the skirts under the jaw and fell against employee, squeezing him between the wall and plate. Employee suffered soreness and stiffness.
Employee was shoveling stone onto conveyor belt and felt discomfort in his back. He did not wish to seek treatment at that time.
Bearing out of return roller on conveyor allowing the roller to become dislodged from the pin. Roller fell approx 20' grazing employee's hard hat and left shoulder impacting hand on hand rail. Laceration to palm of left hand requiring seven stitches.

Employee was working with another employee to roll a 12' beam from the H position to the I position. The employee pinched his right thumb between the beam he was handling and a beam they already had standing in the I position. His thumb was fractured and stitches required.
Reeling up a hose with an automatic rewind. The hose end struck/twisted employee's left hand.
Team member was washing material off the concrete pad when he slipped and fell onto his left knee. He declined immediate medical attention. On November 18, 2013 the doctor diagnosed his knee as a sprain and employee returned to work on November 18, 2013 on restricted duty.

EE complaining of lower back pain. He reported that while operating a front end loader in ship cargo hold #1, the loader struck a protruding object on the wall of the hold. The loader came to a sudden stop and jarred his back. Strained Back.
Employee was sitting in a chair when he fell out of the chair and felt pain in his right wrist.
The man was installing roof bolts in a crosscut. As he started to drill a bolt hole, a piece of rock fell and struck the man on the foot.
An employee was attempting to remove a plug from an 8 inch (203mm) slurry line using a 48 inch aluminium pipe wrench when he felt/heard a ""pop"" in his lower back.
Employee tripped on a welding chipping hammer that was stuck in the grating falling down a flight of stairs causing contusions/strains to right knee and shoulder.
Miner was making repair. He was turning a wrench in upward direction when he injured his back.

Employee was installing lifting hydraulic ram to lift balling drum for repair. Employee felt a twinge in his neck and reported the incident eight days later.
Process maintenance operator was walking down the liner into event pond when the operator slipped, fell and struck his right shoulder on the fuel can, striking the ground.
Lifting corner of bridge crane with 50 ton hydraulic cylinder (port-a-power) to remove cribbing from under crane. As the crane was being lifted, the cylinder was ejected from under the crane and hit employee in chest. Contusion to chest, heart, lungs, spleen, laceration to liver and broken rib(s).
Employee was standing on a dozer frame that had been removed from the dozer, blade was laying front down with arms vertical. When she stepped off the top of the frame onto a hoist pivot cylinder link. The link shifted causing her loss of balance and fell to the ground approximately 42 inches. She landed on her left arm resulting in a distal radial fracture.
Employee was walking out to replace the battery in unit 197 when he stepped in a small hole jarring his back. With the battery in hand and tweeting to the right, he stepped down off the step he was using, his back gave out. He went to the ground in pain and had trouble getting up. Employee sustained a lumbar strain.
The employee was operating a twin head roof bolter. As he was dropping the boom he inadvertently dropped the pod on his foot. X-rays showed no fractures but a contusion to the foot.
Employee was backing up a loader in the hold of the ship. The loader bumped into the wall, resulting in neck pain. Employee placed on restricted duty 11/15/2013.
Employee cutting open a tube of caulk with a utility knife and punctured his right thumb requiring sutures.
Employee was adjusting the tightness setting on the Nordberg Portable crusher. He was reaching for a wrench with his left hand while resting his right on the hood. The hydraulic pressure released, pinching the tip of his right middle finger between lower adjusting nut and washer.
While clearing scale from the Thickener another employees sledgehammer hit an employee in the knee.
Employee cut his finger while using a portable grinder.
Employee was working in the Concentrator on structural repairs. He was unloading a piece of steel from a cart onto the floor grating. The diagnosis was a distal tendon rupture of the right arm.
Employee had finished loading ANFO and was in the process of wrapping up the on/off control valve hose. Employee's feet got caught in the hose and he fell to the ground. Employee was evaluated and diagnosed with a right upper arm contusion.
When dismounting fork lift employee stepped on a board twisting his left knee.
Electrician was walking down the North 42 Inch conveyor catwalk, slipped and fell backwards. He landed on his left elbow and felt pain in his left shoulder. He was taken to the doctor the following day (11/08/2013) and placed on light duty (no overhead work) and given pain medication.
Maintenance employee was washing the lube truck, when he finished washing one side of the truck he started walking to the other side and tripped on a piece of grating. The employee fell, but was able to catch himself with his hands before hitting the ground. He sustained a lumbar strain/sprain to his lower back from the fall.
Process operator was repairing broken nozzle on 4"" pipe (approximate weight 28 lbs), pulled pipe away from rock and felt pain in the left bicep.
The ee was moving a 4 x 4 x 10 piece of tubular metal. While attempting to place it next to a spare cobber drum, the magnet on the drum pulled the tubular piece to it. The ee's left thumb was caught between the 2 pieces of metal. He was evaluated by a Physician: the thumb was fractured and there was a suturable laceration. He was released back to work with restrictions.
EE was removing trash from an office waste basket that seemed to be struck. She yanked the plastic bag, freeing it. Then the bag and the contents quickly came up. Unable to control this action her left wrist struck the corner of the metal desk chipping the wrist bone.
Mine ops employee was leaving the rear house of a drill, they were shutting the door behind them and got their small finger on their left hand pinched in the door mechanism.
Employee was replacing roller on conveyor. During installation of the new roller, the employee attempted to strike the roller in order to tap it into place. In doing so, he missed the roller can and hit his left index finger causing it to become lodged in between the roller can and frame. EE suffered severe laceration to left index finger and received eleven stitches.
Lost balance after lifting 55lbs of flocculant. Injured back.
Employee was installing a roof bolt in #1 entry, when he lost his footing and fell into the joystick. This engaged the rotation and his right hand was caught by the bolt. The resulting injury was a small laceration and ulna fracture to the hand.
Employee was working in a dozer when he decided to exit the vehicle. At this time he caught his left foot on the foot rest and twisted his left knee. Employee hurt his knee on the 5th and sought medical attention on the 6th of November.
Employee was bent over pushing the crusher door shut when he felt a twinge in his lower back. Went home iced it and when he woke up this morning he couldn't stand up straight.
EE smashed pinky finger on left hand between train car and scaling bar trying to remove a boulder. Company doctor removed the nail.

While walking around rubber tire dozer, employee twisted his left ankle on a rock.
EE was attempting to connect a short section of flexible four inch bulk hose to a rail car. The hose section was short which made it less flexible. When ee attempted to bend the hose into the connecting fitting, the hose recoiled causing ee to fall to the same level (ground).
employee was standing on a ladder-the ladder slipped and the employee dropped a metal door on his right hand index finger. after x-rays were taken it was determined that the finger was broken.
While tightening a fitting on a scrubber, the employee pulled a muscle in his upper back.
Injured employee had finished a task at the Coal Mill then stepped backwards, turned around, and lost his balance/footing resulting in a fall to the ground. Injured employee caught his fall with his hands and suffered a closed fracture of the carpal bone to his right hand's little finger. Injured employee's medical treatment includes a splint and restricted work (no lost time).
Employee was attempting to get out of his car when he slipped and struck his ear on the corner of his car door. Employee later required surgical debridement and 6 stitches on 11-15-13.

Employee was holding the base of a ladder for an employee when a chunk of debris fell from the ceiling and struck the employee's forearm. The employee reported pain in the foreman where he was struck with debris.
Pouring cement cell in mechanized heading. A pump was not working correctly and the employee was going back and fourth through a water and cement mix over his boots which resulted in burns to both feet.
Injured Employee (IE) was removing coating from the kiln throat using a rivet buster. He aggravated his old back injury by binding and pushing the coating down with the throat.
Employee was cutting the plastic binding on some filter cloths and slipped, cutting his left index finger.
Two employees were moving crib block out from under head on miner. Employee went to pick up crib when the other employee tossed another crib hitting him on the left pinky finger pinching it between the crib he was picking up and the crib thrown.

Employee was lubricating the bead of a Michelin tire for mounting and received a laceration to the middle finger of his right hand from a loose piece of metal sitting somewhere in the bead area of the tire.
Injured was changing the blade on his utility knife and during the process of changing the blade cut his index and second finger on his left hand. The laceration required stitches.
EE was making repairs to a D8 Dozer. As he was dismounting the dozer, he slipped on the dozer step and cut his right shin.
EE was helping mechanic make a splice in a car cable. When he was cutting the outer jacket off with his belt knife, the knife slipped cutting his left hand resulting in a laceration that required stitches.
While employee was going over to restart outside belts he slipped on snow covered ground losing his hard hat. While getting up he hit his head on a belt scrapper.
Employee was getting into his dozer and slipped causing the door to slam shut on the very tip of his middle finger breaking it.
Employee was repairing a brattice on No. 2 unit. When he turned around he tripped over a concrete block. As he fell his ankle twisted resulting in a hairline fracture.
EE was trimming a window blind with a utility knife when it slipped and stabbed him in the left thumb.
Employee opened a storage door on an underground crane to retrieve a strap. When he reached for the strap the door closed on his left hand. On 12/2/2013 the employee had the hand X-rayed which revealed a non-treatable hairline fracture to his left pinky finger.
Loader operator received a scalp laceration when the loader was digging some hard material, causing the loader to jolt slightly, hitting his head on the sun visor.
EE was sweeping off shelves and counters and got debris in right eye. EE went to dr and received prescription eye drop.
Employee was striping wire with a razor knife and while cutting the wire the knife slipped and cut through his leather glove. The employee received a laceration to the left thumb requiring six sutures to close the wound.
A frozen chunk had plugged up a conveyor on the crusher. The employee used a pinch bar in an attempt to reposition the chunk. The hook at the end of the pinch bar slipped off of the frozen chunk which caused the straight end of the bar to hit the employee in the mouth, chipping two teeth.
Employee was replacing the floor board on the flat bed section of the truck and was welding shims underneath the floor of the bed when he got debris in his eye. Employee did not seek medical treatment until 11/27/13.
Employee complained about shoulder strain. Employee went to the doctor and has to attend physical therapy.
EE was working on miner panel, Panel slipped and fell and caught right index finger between panel and right knee causing him to cut right index finger. Had to have stitches in his finger.
Employee was holding a piece of belt structure another piece fell striking his hand. He has since had to have a skin graft on his finger.
Swinging a sledge hammer on some ice buildup over the conveyor base when he lost grip of the hammer, causing his left pinky finger to be smashed in between the hammer and the frame. he reports the bottom half of his fingernail is ""popped back"" with a ""blood blister"" noted to the side of the affected finger.
Employee was working on a Needle Scaler and was cutting the needles with a cut off wheel. The cut off wheel grabbed and jerked causing a laceration to the employee's (L) forefinger.
Removing conduit coating with utility knife. Knife slipped and cut finger.
Employee was cutting belt when the blade broke and his hand glazed over top of the blade resulting in a deep cut to his right little finger. Four stitches were required, Employee returned to work on his next schedule shift.
Using a wedge to remove liner and piece chipped off and flew up and cut through the pants and cut the knee cleaned area but after 2 days area got infected and to cut and cleaned out
Ee was installing a light fixture. He mis-stepped and grabbed the light fixture, cutting his right hand on the sheet metal frame. Stitches required and released to full duty.
Employee cut left thumb with a box cutter. Employee was holding box with left hand while opening it with a box cutter on her right hand. The box slipped and the knife made contact with her left thumb requiring stitches.
Employee was cutting a piece of belt, while cutting the belt the knife slipped and employee cut his leg resulting in four stitches.
Crusher Operator attempted to check the voltage with a volt meter at the electrical panel. After opening the panel door found existing lights not on. Picked up the volt meter to check and an arc flash occurred causing burns to the back of his hands.
Employee was grabbing cable spinning hoist and his little finger -(RH) got stuck in hoist.
Employee was bent over installing a belt guard and hit his head while raising up causing a sprain to his head and neck. He did not miss any work. This accident was not reported to the company. The company was not aware of this accident until we were contacted by our Workers Compensation provider.
Employee was opening a mandoor when the sudden rush of air pulled his left arm and shoulder. The injury occurred on 11-22-13, but it wasn't until further medical evaluation that it was deemed that surgery was needed on 12-13-13. The employee has missed no work days as a result of this injury.
Two employees building cribs in 4-East area of mine, as one employee tightened his crib, the sledge hammer handle hit the crib, bounced out of the employee's hand causing the sledge to fly, striking an employee on the left side of his face causing a 3/4-inch laceration on the left side of his face between his lips and nose requiring sutures to close the wound.
Employee was cleaning his hands with a shop rag that had slivers of metal in it. As he wiped grease from hands a sliver of metal from rag entered his right thumb at the joint.
Employee was helping two welder repair crusher during a schedule shutdown. The EE was exposed to the light from the welding operation that was going. The EE had the access to the correct PPE but chose not to wear it. The EE was seen by the local Dr on 11/22/13 and giving perspiration medicine for his eyes. The EE is doing fine and returned to work the same day.
Employee cut the outside of his hand when using a portable drill.
EE states that as he was examining the regulator he observed that the roof needed scaled. As he scaled the roof a piece of roof that measured 15""X10""X2"" thick, struck him on the top of his forearm. EE finished his shift and went to Med Express where an X-ray was taken and no injury was noted. EE continued to work and perform his regular duties, Please see attached page.
EE fell to ground bruising the left elbow while working underground-finished shift.
Using a wire brush to clean and got debris in his eye. Required prescription for eye.
Employee noticed injury to his eye when he woke up the morning of Thursday 11/21/13. He was unaware of any incident that lead to this injury. When notified safety officer and foreman noticed a spot on his right eye. He was driven to the Mayes County Medical Center for further examination. He was then sent to Claremore-Pryor Eye Clinic and given antibiotic eye drops.
The employee was guiding a top idler frame into place with a sledge hammer and inadvertently struck his own left middle finger causing a laceration. Medical Treatment only.
The injured was lowering the man-up on a roof bolter and smashed his finger on the frame of the machine.
Rock from an overhead conveyor fell striking EE's safety glasses; causing a contusion of the lower eyelid of the right eye.
Employee has discovered apparent bite marks, both red and bumpy in color, similar to that of a mosquito bite. Employee has found these apparent bites on both forearms and on the #4 finger on the left hand.
Charging battery or doing something with battery and it blew up in face.
The employee was working the Progressive Cavity pump slowly with a pipe wrench to clear fudged product. During this process the pipe wrench slipped catching his finger between the handle and floor plate.
Employee cut his right index finger descending ladder on 777 Cat Truck.

A mechanic was removing a bearing from the ball mill discharge auger shaft with a hammer and a shard of metal stuck another mechanics right arm.
A Roof Bolter Operator Trainee, while being trained, got his fingers caught between the drill steels as he was putting the drill steels together to drill a hole. Employee was wearing the proper metacarpal gloves at the time of the accident. Laceration to his finger required stitches.
Employee was lifting a 50# bag of carbon to dump into the blender feed bin. He lost grip of the bag with his right hand so all of the weight was supported by his left hand. He felt a pull in his left arm. Later that night he continued to have discomfort in his left arm. He went to the doctor for evaluation.
Employee was helping to tighten a bolt on a loader when the adapter broke. The employee slipped and fell to the ground twisting his knee. The employee reported it to the office. After the Thanksgiving holidays he came back to work and said he needed to go to the doctor about his knee. He was taken to the doctor and they recommended he have physical therapy.
While unloading crib blocks, the employee was straightening the blocks and had his finger caught between two of the blocks.
Maintainer was installing a pin in the field. A sliver of metal broke away from the pin and penetrated the left lower leg. Once the accident/injury occurred mine safety was notified. The injury was sustained as non-threatening. The maintainer and his helper closed out their job and went to Collins Medical and was treated for minor laceration and received stitches.
Employee was disassembling a D11 roller frame using an impact wrench & socket. The impact socket busted cutting employee's left hand.
The IW was removing tile from a wall during a locker room remodel. The piece of tile slid from his hand and fingers down his arm lacerating his arm, requiring stitches.
Injured was trimming top out of the #3 face on the #1 unit when a piece of carbide struck the injured in the head causing a laceration which required stitches.
A rock was placed in the jaw. The rock needed to be moved. Employee used a bar to move rock. While doing this he strained his lower back.
A windy day. The employee was walking to the parking lot at the end of the day to go home. He still had his PPE on. A gust of wind blew a speck of dirt into his eye. He went to the office and flushed his eye. He then drove home.
