A roof fall was discovered at approx. 11:10p.m. in the 2 West Mains travel way. The fall occurred in the #3 entry of Spad 1062 and measured 40' long X 20' Wide X 8' long.
A roof fall was discovered in the Left Return of Byrd 2 Mains. The fall was in the #1 Entry between survey stations 1388 and 1372. The roof fall does not impede ventilation and is not a travel way. The area had been timbered off to travel.
A power failure at 06:42 caused the main hoist to trip out. The cager was in the cage at the time. Hoist was brought up on generators by 07:13 No injury to the cager. The hoist was brought back into full service by 8:28am once the power was restrored to site.
The overloads kicked out on the main hoist while bringing miners to the surface at the end of the shift. The overload was reset causing the hoist to operate normally.
There was a plugged condition in the discharge area of the Rotary Hearth Furnace which we believe resulted in uncontrolled combustion of CO. There were no injuries and no damage to equipment. This does not constitute an admission that an ignition occurred. This report is filed out of an abundance of precaution.
Limit switch at the tipple on the production hoist failed. Hoist would not work in automatic or manual. There were not two ways in an out of the mine. All underground miners evacuated with no issues or injuries. Hoist was back in service at 01:30AM.
A rock fall that was 4.5 feet thick by 16 feet wide by 20 feet long was found in the number two entry one cross cut inby the portal. This fall does not effect the ventilation or travel of miners. The fall was dangered and timbered off.
Sometime during Christmas break a rock fall occurred in the left return at just inby SS# 2577. The fall was dangered off, timbered off and the travelway was re-routed around the fall. The fall was approx. 80' long, 20' wide, and 7' high.
DC drive failed causing Metz elevator to surge tripping the safety catch and bending the linkage rod between the lower and upper safety catches on right side of car. Elevator repaired by 2:19 pm. No entrapments.
A 24vdc to 120vac converter went out. Had to change out faulty invertor.
ALLEGED IGNITION - There was a 103J order #8526271 issued on 12/24/13 for an alleged ignition on #1 Longwall. After an investigation by MSHA Inspectors, UMWA and Mine Operator it was determined that an ignition did not occur. We are submitting this only because a 103K order was issued and has not been vacated as of today and to be within the allowed 10 day reporting process.
Inundation of Mine area due excessive rain in area
At 1:26am on December 22, 2013 the TRJV Mine experienced a power bump, that was later determined to be caused by problems with the NV Energy transmission lines . The bump tripped out the #2 man hoist, the primary access to the underground. Electricians reset the hoist at 1:52am.It took until 1:56am to run through the proper tests & put the hoist back in to service.
An unintentional roof fall has occurred on the #2 Section #5 Belt #4-5 Entry Intake. The fall was approximately 40ft x 19ft x 4 to 5ft. No injuries were reported and no equipment was involved.
Employee was walking the air courses and found a roof fall at 21+50 in the #2 entry in the return. The roof fall was in a four way intersection. the Fall was 17 feet by 15 feet and measured 4 to 6 feet high.
At approximately 10:00am on 12/20/2013, Kiln 4 coal weigh feeder was undergoing a belt cut which required the temporary removal of the coal feed to the mill. During this time, the coal dust inside the mill ignited causing dust to be expelled from the feeder opening and the hood of the kiln.
As a supervisor was making plant rounds, he noticed a smell of smoldering / slow-burning coal in an unused coal hopper. No damage occurred, but hopper was hot and coal was in danger of igniting.
Elevator was down due to an electrical problem.
A fall of the roof was discovered on the mainline #12 Belt at 22 crosscut. The dimensions of the fall were 50 feet long X 16 feet wide X 12 feet high. Travel was affected in the area.
During mining operations on our #2 MMU a methane ignition, bleeder fire, occurred at approximately 1:15 pm. The bleeder fire was approx 3 feet long and burned for about 30-40 seconds. MSHA was notified at 1:25pm and a plan was developed and approved by the District Manager addressing this incident. No miner was injured due to this bleeder fire.
Unintentional occurred on the 7th 48 beltline x-cut 12 , #5 entry 12' length x 12'wide x 6 thickness . ( This area was pinned with 72"" tension rebar.)
Continuous mining machine had at least 5 bits worn down into the shank with no metal carbide tip present. Ventilation plan not being followed. Fire suppression system not in secure location.
Tracking system was down due to an optic fiber line being cut.
A unintentional roof fall occurred between #5 & #6 entry and NE mains at #12 x-cut in the neutral, measured 80' long x 19' wide X 8' high, cribbed & dangered off. Area will not be rehabilitated.
Operator received an alarm in the ccr indicating that coal mill vent doors opened. No personnel injuries or property damages. No sounds heard from workers in burner building or ccr. kiln operating parameters, coal mill inlet and outlet temperatures were normal at the time of the event. Small amount of smoldering coal was found to be present in the inlet duct.
A roof fall in the track entry 11 breaks underground it measured 6'foot thick by 30'feet long by 30'feet wide
A roof fall occurred in the 3rd Southwest Mains in the intake travel way at crosscut #95. The fall occurred in an intersection.
Length 35', width 18' thickness 7' located on 8th 48 beltline, x-cut 30.5 , #5 entry.
A roof fall occurred at 79+25 in 4 west section in #2 entry inby the tail. The fall measured approx 32' in length, 16' wide and 15' in height. Area was bolted with install III resin assisted bolts, CC cable bolts in every other row along with screen and bacon straps. Contributing factor may have been physicals characteristics of strata and low cover.
A roof fall had occurred at 58 1/4 xc in 3 west in #4 entry. The fall measured approx. 10' in length, 17' wide and 5' to 6' in height. Area was bolted with 8' install III resin assisted bolts with screen, straps and 12' CC cable bolts installed in every other row on pattern. The return/alternate escapeway where re-routed and re-opened. Caused was deterioration of the mine roof.
A 24 volt breaker tripped, resulting in a couple of small CO combustions. There were no injuries and no damage. This does not constitute an admission that an ignition occurred. This report is filed out of an abundance of precaution.
The DC converter control card for the production hoist failed not allowing the hoist to function.
Black smoke observed coming from fluidizing fan of dryer. Dryer was not running. Due to unsuccessful attempts to start the dryer, fuel oil inside of combustion chamber was smoldering. Issue was contained inside of combustion chamber. Fire department called as a precautionary measure. MSHA was notified within 30 minutes of the unplanned event.
A rock fall was discovered by a pump man in the 3 V panel. The fall was on the cable leading to the pump. 3 V panel (3+70) Entry # 2. 19'in length x 19' in height x greater than 6'.
Roof fall at 19 cross cut 5 belt across 2-4-5 entry.
Slope hoist operator observed that there were some broken wires on the hoist rope, just above the connection to the brake car. This damage was observed while preparing to make a connection to a load of supplies. The hoist was immediately taken out of service and MSHA was contacted.
At approx. 4:10 pm, a roof fall above the anchorage was discovered in the 4North intake escapeway just inby spad no. 52+65. The fall was approx. 40ft long 16ft wide and 6.5ft high. There were no injuries or property damage. The fall was dangered off and the escapeway re-routed. The area had been supported with 6ft resin assisted bolts.
Electrical power to mine site interrupted rendering all electric hoists inoperative. Adjoining West Mine hoists also affected. U/G miners temporarily unable to exit to surface. Personnel moved to refuge areas. Power and hoist service restored at 10:25 p.m. No injury or illness reported
While ARSI contractor employees were welding on the housing of B4 BC 1 the sparks from the welding landed on the conveyor and it began to burn. The belt is enclosed in a metal housing and made extinguishing the fire a challenge. The fire was extinguished 45 mins after it had started.
Electrical power to mine site interrupted rendering all electric hoists inoperative. Adjoining East Mine also affected. U/G miners temporarily unable to exit to surface. Personnel moved to refuge areas. Power and hoist service restored at 10:32 p.m. No injury or illness reported
Coal Thayer belt tripped causing coal mill to become lean. Mitchell IN was receiving a winter storm, the combination of ice and snow that built up on the vent might have caused a delay in when pressure was released. Vents are set to release at 1 psi. Some smoldering coal in the duct work on the bottom just right inside of door during investigation
There was a loss of feed to the coal mill because of wet frozen coal, which caused the mill temperature to increase. Also, because there was a loss of feed, there was an increase in dust as well, which then produced a pop inside the mill. No one was injured and there was no damaged equipment.
An unexplained roof fall occurred in the neutral entry at 16 crosscut on #18 belt. No ventilation controls, equipment, or travel ways were damaged by the fall. The fall was approximately 70' long x 19' wide x 8' thick.
A miner was entrapped on the 1 North elevator for longer than 30 minutes. While performing an examination the elevator lost power. No injuries occurred and no damage was caused to the elevator as a result of this incident.
unintentional roof fall occurred off the neutral in an area that had already been dangered off between spad 134-145 in #3 Entry between break 7 & 8. Appx 30 ft long, 20 ft wide, 6 ft high.
At approximately 11:46 am the service hoist went down when the magnet for the gate at the bottom of the shaft malfunctioned.
At about 2:27 a.m. smoke was observed near the S713E-Left prod area. Reports and notifications were made to evacuate the mine. MSHA hotline called. Investigation revealed that smoke was generated from belt drive pulley friction on belt. A fire was not observed and no physical evidence of fire was collected. Determined there was no fire. No injury or illness reported.
Our ESG seismic system picked up a rock burst at 3:01 pm on the east end of 15 stope at day shift blasting time. At 5:00 pm it was confirmed by the night shift crew that a rock burst did occur with the day shifts blast and did damage to the east side of 15 stope, dislodging 5ft of the back for a distance of about 16ft at the end of the stope.
A roof fall occurred in inactive works that damaged an adjacent stopping in active works, resulting in impaired F-Seam ventilation. The fall occurred in F Seam, South Main Returns, entry 6, between crosscuts 14 and 15. The fall was approximately 5' high, 30' long, and 18' wide.
At 7:10 pm an ignition occurred at the face of #3 entry between 76 and 77 crosscuts in the D1 section where the 14ED25 miner was cutting coal. An orange flame approx. 2 feet long appeared in the upper right hand corner of the face where there was some sand stone above the binder. The flame was extinguished using the wash down hoses on both sides of the miner afet approx.7 seconds.
Two contractor employees were in the K3 elevator travelling upward. One of the employees accidentally hit the inside door with a broom causing the elevator safety features to enable stopping the elevator between the second and third floors. The men exited the elevator at approximately 3:49 PM. There were no injuries as a result of this incident.
At 10:43 pm an ignition occurred at the face of #3 entry between 76 and 77 crosscuts in the D1 section where the 14ED25 miner was cutting coal. An orange flame approx. 2 feet long appeared in the upper right corner of the face where there was some sand stone above the binder. The flame was extinguished using the wash down hoses on both sides of the miner after approx. 6 seconds.
Employee was operating a D9R Dozer on the coal stockpile when without warning the coal broke loose under the left track of the dozer. The dozer pivoted downward into a void and was encased in coal. The operator was uninjured but he was entrapped longer than 30 minutes.
An unintentional roof fall has occurred in 1st Left Panel North Submains #1 Entry 1 Break inby/outby spad #8719. The roof fall was approximately 90' x 20' x 5'-6'. There were no injuries reported and no equipment involved.
An unintentional roof fall has occurred in 2nd Left Panel North Submains #9 Entry 2 breaks outby spad #9158. The roof fall was approximately 120' x 20' x 5'-6'. There were no injures reported and no equipment involved.
2 of 3 utility owned power lines came loose from pole and fell to ground. The lines continued arcing for over 30 minutes, preventing the fire department from extinguishing the small brush fire. The Utility Repair Crew replaced the lines and system parts up to the (utility owned) pole mounted transformers. No Injuries, or damage.
The #1 hoist electrical breaker was locked out to allow mine personnel to reposition the brake counter weight proximity switch. After repositioning was completed, the #1 hoist electrical breaker would not turn back on preventing the mine from raising and lowering personnel with the #1 hoist. The mine still maintained two means of egress with the #2 and #3 hoists.
An unplanned roof fall occurred in the Wyoming #2 mine at spad #8915 that measured 20' long x 20' wide x 6 to 7' in height. No injuries were reported. The area was developed on 1/16/2011.
A capacitor bank on the main power feed to site malfunctioned, tripping all power to site. This tripped out the #2 man cage stranding 20 miners in the cage, some 100ft from surface. Men were stranded in the shaft until 18:15.
Examiner found a rock fall located 4th South West Submains Outby Unit #1 in the primary escapeway. (22+60) XC # 30, Entry # 4. It was Approx 35' in Length, 19' in Width, Greater than 6' in height.
Employee was standing on a ditch bank when it collapsed. He fell into the ditch and was covered/crushed by falling material
Victim was last observed accessing the stern if barge NBN-611. Shortly after, he was noticed to be missing. Snow was present on the barge. Damage or impairment is unknown at this time.
Individual was hit by fly rock while setting off a shot.
The injured employee's left hand was caught in between the frame and the pivoting idler base in a scissor like fashion pinching the left ring finger between the 2 pieces of steel. The finger tip, about 1/4"" was amputated. The IE was wearing gloves at the time of the incident.
Employee placed his hand on rail for leverage to stand up when rail slid over and smashed his right pinky finger against another rail resulting in (distal) amputation by physician of right pinky finger below first knuckle.
The Shuttle Belt and the HP 300 crusher discharge belt were not de-energized, and the electrical disconnects were not ""locked-out"" prior to work being performed. See citation #879638
Foreman Trainee following 636 Scoop traveling inby, 59-60 block 21D, with 650 batteries on it's duckbill. Foreman put hands on top of 650 batteries (unknown reason, may have tripped and tried to catch himself). Battery end of scoop went down in rut, 650 batteries raised and roofed. Pinched both hands between battery and roof.
While working on the center pin on the left side ""Z"" bar of the Caterpillar front end loader the ""Z"" bar dropped and amputated the employee's left index finger at the first knuckle. Employee failed to properly secure the ""Z"" bar prior to working on it.
While repositioning the dragline cable out of the way to walk, the injured caught his thumb in a strap severing the top of the thumb.
While working on the roof bolter, employee got fingers caught in boom cylinder and drill body chain mount. Boom came down and severed 3 fingers on right hand.
Miner was pulling a hydraulic hose off an excavator. A zip tie had to be cut to get the hose off. While bracing with his right arm the miner attempted to cut the tie with a pocket knife. The knife slipped in the upper cut, stabbing his right forearm.
A forklift was being towed up the slope haulage road by a diesel scoop. The forklift had to be disconnected from the scoop. Another employee asked the IE to check to make sure the tow bar was disconnected. When the IE picked up the tow bar, two 1.5"" diameter clevis's attached at the tongue of the tow bar shifted, smashing his right middle finger against the tow bar.
Employee was walking through tipple and tripped over a step. Didn't fall completely. Initial incident occurred 12/31/13, however employee didn't report incident until after Form 7000-2 was submitted on 1/10/14. Due to delay in reporting incident, reporting to MSHA was overlooked, but was reported to Worker's Compensation. An amended 7000-2 will be submitted.
EE was cleaning out from underneath #11 mine conveyor /shaker on the crushing line with the mud bucket, when suddenly the mud bucket went into reverse pinning him between the mud bucket and and ""I"" beam.
Employee was standing on wheel chock berm fueling and end dump when he slipped. He was pulling on the fuel hose and injured his back. He did not report this till January 3 2013. Strained back.
The employee stated that as he was getting off a man jeep, he stepped down and twisted his right ankle. He sustained a right ankle sprain.
Employee went to the sample building to sweep out the system towards the end of her shift. She walked around the truck, swept out the system, after she got that completed she walked around the truck to get back in. As she went around to the driver side she slipped in the mud and twisted her left knee.
The employee was making a cable splice and started to crimp the burndy clamp. He was using a splice knocker to crimp the sleeves. When he struck the knocker, a piece of metal chipped off and went into his hand. The employee thought that he removed the metal but went to the doctor on 1-13-14 and underwent surgery to have it removed.
Employee was greasing equipment when tarp line caught his boot and he fell injuring his right arm.
Employee was walking on elevated platform 39 ft above ground it appears a section of the floor broke out from under his footing causing him to fall 39 ft to the ground, it fracture his back.
Employee was straining tore move a nut off of a bolt on a steering jack on a scoop when he felt a twinge in his lower back. This became a lost time injury on 4/28/2014.
Production loader operator had his right foot sitting in an idle position while operating a loader with his ankle rolled to the side, it felt like it was starting to fall asleep. He proceeded to walk around the loader to relieve the pain and was unable to.
The injured employee was advancing ventilation curtains in the No. 9 Entry of the East Mains when a coal rib fell. The rib struck the employee in the chest and right hand. The employee was taken to the hospital for x-rays. X-rays were negative. Contusions only. Employee was held out of service for 2 days by the medical provider.
Employee was operating #20548 a D10 Dozer on the dump and Truck #24903 a 789 backed up and struck the cab area jarring the employee. Employee completed his shift and went to Logan Regional Medical Center and was diagnosed with a Thoracic and Cervical Sprain and taken off work till 1/3/2014. Employee was examined by a specialist on 1/3/2014 and taken off work thru 3/3/2014.
Employee was coming out of changing room. When he stepped out of the door he fell and twisted his right knee.
Employee was going to move drill cable out of road way. He was standing in loop of cable. The operator turned the reel on, knocking his legs out from underneath him. He fell and landed on his right hip resulting in a bruise.
Claims he pulled a muscle in his abdomen while pulling on a pipe.
Ee was coming down ladder on equipment. Went to reach down to next rung and heard pop in shoulder, felt immediate pain. Seen at the ER and referred to orthopedist. Orthopedist is requesting diagnostic testing for cartilage tear. Released to restricted duty that cannot be accommodated.
On 12/29 employee alleged that during his previous shift (12/28) he was jostled while shifting a dozer from forward to reverse, resulting in back pain. The employee was evaluated by a doctor on 12/29 and issued Rx muscle relaxants & released back with restrictions. He was unable to report for 2 shifts due to the side effects of the muscle relaxant. He returned full duty on 1/1/14.
Contractor was exiting a Bobcat when he stepped on to the face of the jack hammer and slipped. He rolled his ankle, resulting in an ankle sprain. No meds prescribed. No modified duty available.
The roadway was stoned, but still not clear entirely for normal travel. Traveling on the roadway, the driver of the vehicle lost control of the vehicle on the snow and caught the edge of the blacktop roadway. He tried to correct the vehicle and the articulated box of the truck flipped on its side.
Employee was checking on a scoop tractor. The section was moving power and the miner cable was pulled on inby pulling the miner cable down on the injured neck.
The injured employee was replacing an engine on a drill 121. He was lifting a fuel filter base when he felt pain in his lower back.
Traveling uphill and truck lost control and overturned on hill.
While working on casing in a well pipe, the pipe slipped on its brackets and came down on the employees' right hand.
The Employee bent to pick up a concrete block and felt a pain in his lower back.
Prying a rock out the feeder with a metal bar and strained back.
Employee was walking when employee tripped over a piece of clay causing him to fall and strike his left arm and shoulder against the rib. Resulted in a dislocated shoulder.
Injured employee was using a grinder to cut a wire rope at the 1 North Belt take-up area when a piece of debris entered his left eye causing irritation. Injured employee was wearing regular safety glasses at the time of the incident.
Injured employee sustained a laceration to his left wrist/hand while operating a roof bolter on 1 North Mains. The employee was inserting a 12' cable bolt into the hole when the plate caught on the bolt. He used the bolt wrench to strike the plate when the plate came loose and moved down the bolt striking his hand. Three sutures were required to close the wound.
Employee was applying plaster to a stopping when he stepped back causing him to slip and fall backwards onto a cement block injuring his back.
Employee was drilling a hole for a rib pin on the left side of the 2D miner. When trying to remove the auger the employee pinched his left pinky finger between the auger and the drill chuck when he activated the control. The injury resulted in a laceration and fracture.
Employee fell stepping out of the Raw Coal Control Room, injuring his Right knee.
Conditions were wet and muddy. Injured was attempting to get on swivel truck and right foot slipped while left foot was on non-skid surface. Lost balance, weight shifted, twisting left leg and resulting in fall from truck. Was taken to nearby clinic and preliminary diagnosis is possible torn quad tendon / muscle.
Employee slipped on mud and fell (same level) hurting his right upper thigh.
Employee was walking on top of a tram road rut when the rut gave out causing the employee to roll his right ankle. Employee began missing work on December 26, 2013.
Carrier bearing that holds drive line (shaft) broke allowing drive line (shaft) to move rupturing air line that goes to the brakes causing brakes to fail. At this time the truck (International Truck #15) begin rolling backwards. Employee jumped out of the truck landing on his back injuring shoulder (right) & his back.
Mechanic was in a sitting position when a co-worker walking by threw a piece of rock at him striking his right. Right hand sustained a simple fracture of a metacarpal of little finger.
Employee claims to have dropped a empty pallet on foot. Employee worked from 3:30 until 4:30 and did not report the accident. Employee reported accident following morning via voice mail. Accident resulted in a fractured toe. Employee missed one day of work and returned with no restrictions
Employee drove thru a run thru check on a utility vehicle and struck the boom of a loading machine parked on the other side causing injury to his right upper chest area.
The IE employee stated he was driving a connector link on a conveyor chain on the #18 continuous miner. He stated he was using a 15lb. sledge hammer to drive the connecting link to the strap. While striking the link a piece of metal struck the employee in the right arm approximately 3"" above the wrist. IE had surgery to extract the metal from his arm.
Employee was operating a forklift on PA st. rt. 231. The fork lift dropped off the rt side of the road. Employee could not stop in time & the fork lift rolled over. Employee was wearing his seatbelt, but when he realized the vehicle might upset, he removed the seatbelt and attempted to jump clear. He landed on his left elbow and rib cage, bruising both.
Unknown. Employee reported after leaving work his shoulder hurt. No other information is available. Nature, time, place and circumstance around injury are unknown. Employee does not know how injury occurred.
Driving a haul truck when he hit a pillar
Employee was setting up diamond drill when he dropped his hammer, bent over to pick it up & was struck in hard hat by a piece of timber he was setting up. Doctor issued restrictions.
Repair man changing boom swing jack on continuous miner. Trying to loosen hose fitting when wrench slipped and cut left hand. Went to doctor had four stitches. No time lost due to injury.
Standing on back of transport tractor getting ready to dismount when the driver's foot slipped on the clutch causing the employee to fall from the tractor which resulted in a fractured hip.
Miner was engaged with guzzling out a vessel in the strip area. Miner received some very minor caustic burns on the neck and his face. Miner rinsed off for several minutes. Taken to hospital flushed more. Released full duty. On 12/22/2013 miner returned to hospital was given three days off by ER doctor.
Employee was walking into the hopper building and tripped on scale. He tried to catch his fall and tore his upper right bicep.
Employee was picking up a cross over pad while his partner was moving roof bolter. As he attempted to put the pad onto the miner, he felt a pain in his lower back.
Employee was lifting pulling a piece of belt structure when he felt a sharp pain in his neck. Pulled muscle.
Dismounting excavator slipped off step had hold of hand hold pulled shoulder
An employee of Love Mechanical Cut the back of his leg with his pocket knife while cutting a rope. The cut required stitches.
Individual was working with the track crew removing haulage rails from the longwall section. He was standing in front and to the side of a rail being pulled with a steel rope. The clevis and cable broke loose from the end of the rail and struck the individual on both lower legs causing a fracture to the tibia bones in both legs.
While using electric appliances employee rested her left hand on microwave while pushing down the toaster level. Employee felt a shock go through her left hand and arm.
Employee was working on R -3801 with other employees attempting to correct a belt that was folded under and pinned against conveyor rollers. The employee stated he was lifting up on the belt, when he felt pain in his right arm bicep region. The employee was taken to St. Francis Occupational Medicine for treatment
Employee was pulling high voltage cable out of cable car when he said Oh I hurt my shoulder to witness. Kept on working. EE said he told foreman, foreman said he didn't. I was told on 12-19-13. No one can tell me what date injury happened.
Individual was lifting a metal plate on the Alpine Mine when experiencing a pull / pop in the left shoulder. This injury did not involve any compliance issues, malfunction of equipment not involved, did not involve job skills, training or attitude. Individual continued to work until 9/11/2014 when surgery to the left shoulder was performed.
Employee was on ladder installing spads when he began to drill, a piece of rock broke and fell with the drill still in it pulling employees arm down with it causing pain in shoulder. Employee has been working since date of accident and has now been taken off work. Employee did not start losing time until 2/18/14.
A timber fell striking employee's left hand and mashing his ring finger against belt rail resulting in 8 sutures.
Employee was driving the loader when he drifted off the road when he was hanging up his microphone for the CB and when he drifted off the road he hit a rut in the road and hit his head on the rops cage in the loader.
While lifting rotary feeder tips, the employee felt a pop/pinch in the lower back and is diagnosed with a lower back strain. This incident became reportable on 1/21/2014.
Employee was walking to the face, to reset a pump. He stepped on a pile of gob sinking to his knees. He fell sideways, twisting his left knee.
Employee stated he fell from Komatsu 300 excavator while unhooking tarp and thinks he broke shoulder. 12/19/2013 employee confirmed dislocated shoulder, broken neck. No surgery required. He's to wear neck brace for two weeks and arm sling.
employee was working on scoop when rock fell from between roof bolts and struck him on the back of head/neck causing small laceration. 3 stitches were required
Employee was removing the male plug from a set of scoop in preparation to charge the batteries. As he pulled on the plug he felt a ""pop"" and pain in his left shoulder.
A miner engaged in the replacement of wear components in the impactor of a crusher. As one of the components was loosened, it shifted pinching the miners ring and pinkly finger between it and the machine frame. The injured was given first aid and taken ti the hospital where he was given four stitches and released. Miner is scheduled to return to work on Jan.
Employee was assisting the continuous miner operator in moving from place to place. As he was walking back toward the coal transporter, he tripped on uneven mine floor and fell into the coal transporter. During the fall, he injured his left shoulder causing a fracture.
Sprain left shoulder lifting an alternator out of a 475 dozer. Normal conditions.
Employee was removing rocks off the toes of shields at #331 shield. He had checked roof in the 1 foot shield gap and had picked up 1 rock to move toward the pan line. Another stone fell from between the shields striking the first stone. His hand was under the first stone which pinched fingers between the stone and metal. Several fractured fingers.
Slipped and fell while walking over hard packed snow.
Electrician /Mechanic kneel down and turned around and felt his knee hurting. EE had prior surgery on his knee before employment at mine. EE said that he had forgotten to wear his knee brace. The EE said he would RTW the next day. He did not go to hospital after leaving mine property.
EMPLOYEE HAD APPROACHED SCOOP THAT WAS PULLING A LARGE PORTION OF RUBBER HOSE. THE SCOOP WAS POSITIONED JUST OUTBY THE OUTBY RIB OF THE CROSSCUT AND HE WAS POSITIONED ON THE INBY LEFT CORNER OF THE CROSSCUT. AS HE TALKED WITH SCOOP OPERATOR THE STRAP HOLDING THE PIPE BROKE. THE PIPE WHIPPED INBY STRIKING EMPLOYEE CAUSING SERIOUS INJURY.
Shop ee was checking roller and slipped on piece of rubber. EE informed supervisor that he was not injured and refused first aid or medical treatment. Safety or management were not notified that it was a recordable and required treatment until the evening of 1-20-2014. Ee is being referred to a physical therapist for physical therapy-treatment of an injured ACL.
Miner was walking across the lot when he slipped and fell on ice, spraining his back shoulder and right hand.
While operating a walk-behind concrete saw, a rock jammed the saw wheel. Employee reached to remove rock and his sleeve was caught on saw arbor causing forearm to contact blade.
Employee was stepping from the back of his service truck when he missed a step and fell. He attempted to brace himself with his left hand and that is when he suffered a sprain to his left wrist
Employee was jacking on wreck car on 8 section track when the jack mechanical slip causing employee to fall backwards injuring his right shoulder/elbow.

EE alleges he was removing bolts from the wash screen with a driver guard and he turned to switch positions and he twisted his right knee.
Loading a piece of iron with a bobcat, the iron caught and fell off bucket onto the workers left ankle, causing a break
Driving 2B motor down 9 South haulage when trolley pole came off the wire. Employee felt pain in his left shoulder putting the pole back on the wire. Started missing work on 12/18/13 for sprain/strain in left shoulder.
Employee stated after he backed the continuous miner out of a finished cut he turned to walk away and stepped on a loose rock lying on the bottom and twisted his right ankle
The employee was pulling cable off of the cable reel on 409 roof bolter when he felt pain in his groin area. The employee was diagnosed with a hernia and had surgery on 12/20/2013. Medical information that was received today supports the injury was the result of a work related injury.
Employee was taking a set of center miner drums apart in the mine yard when drums rolled over on his left foot fracturing five toes.
EE was getting ready to weld a stop of for an overhead crane. EE fell 20 feet to the steel floor below landing on his left wrist and knee. EE was transported to CMAC General and was treated for a broken wrist and knee. EE was released on 12/19/13 and is now home.
Employee using a volt meter to check the breaker on a belt on the crusher. The breaker blew and caused a flash burn to right hand; middle,pinkie and thumb.
He was walking from his car to the showerhouse prior to his shift starting. He slipped on some snow and landed on the ground striking his buttocks on the ground. On 12/26/13 medical information was given to this office showing his fall to the ground may have caused a strain to his cervical spine.
Operating a loaded tractor trailer coal truck the drive wheels were spinning and a weak transmission mount allowed the shift lever to move excessively smacking the operators hand starting missing on 12/20/2013
Mining overcast in #8, inby 329 block, some roof rock fell on the miner head. Miner operator was prying rocks off of miner with a slate bar and felt pain in his groin.
EE slipped on ice and injured lower back.
Injured employee was performing his normal belt tending job. Some of the conveyor belts stopped running and employee had to turn the LP gas off which was heating the conveyor belts. He got out of the truck and started towards the shutoff valve and then slipped on the sand, snow, and ice with his right foot. His left shoulder struck the vertical beam under the BC03 stairway.
EE was tightening a nut that secures the ceramic plate to the drum. The ratchet spanner slipped off the nut and cut the employee's wrist on a stainless steel edge. The employee sustained a laceration to his wrist and stitches were required.
Injured was trying to turn the swivel of a portable light plant into position and when it came loose, injured's right little finger struck the light plant mast stopper causing a cut to the finger tip.
Descending from dozer; slipped on ice covered tracks.Struck R side on the dozer push arm, pulling muscles in back & left arm; numbness in leg & neck.
Climbing down a ladder in the #6 elevator from the top of the elevator after making an inspection and his leg buckled causing him to fall off the ladder and strike the floor. He strained his right shoulder and fractured his left ankle. He began losing time on 12/16/13.
EE was cleaning bath house. He attempted to step over a bench while pulling a water hose in the change room. He hung his toe on the bench and felt a pain in his knee. He was diagnosed with a twisted knee. This accident became reportable on 02/21/2014 when the employee started missing work to have surgery.
Employee was operating forklift loading kaolin onto a trailer. As employee was backing off trailer, trailer shifted away from dock, causing back wheels of forklift to become lodged between trailer and dock. Jolt of the forklift caused the trailer to shift further resulting in the forklift falling approximately 5 feet to the ground with employee on the forklift.
He was carrying belt structure near the 5 North #4 Beltline. He slipped and struck his hand against a belt roller causing a laceration. The laceration became infected over time and required minor surgery to remove the debris.
Employee was climbing off of excavator when his feet slipped on the muddy tracks and he slid off onto the ground hurting back of neck and left shoulder.
Employee felt discomfort in his left shoulder while loading a nitrogen bottle up into a nitrogen bottle carrier.
Walking to lock out main mine fan door, thick ice under the snow. Employee slipped and fell twice while traveling to the fan door causing pain in his right shoulder.

TRIPPED ON STEP LEADING INTO GUARDSHACK
Employee was walking to job site when he slipped on ice and fell on his face.
Employee stated that he was walking into the warehouse and as he passed through the door he felt pain in his left knee.
Employee was walking from the track entry to the belt entry in a crosscut and fell down landing on his shoulder. Began losing time on 1/21/2014.
Employee slipped while walking to vehicle parked in small fleet vehicle area.
Employee was pulling slack in miner cable and felt pain in his back. He continued to work up until 1/2/14 when he was taken off work.
On 12/12/2013 at or around 1:00 AM Employee was climbing up the step of the 4001 Haul truck, he fell forward striking his right knee on the bumper of the truck. Employee continued to work as the days passed his knee kept bothering him so he went to the Doctor on 1/9/2014 Doctor placed him on light duty, starting 1/13/2014.
Employee was in the process of installing a rib pin at the #2 face when he bent the drill steel he was using, while trying to remove the bent drill steel from the drill pot the employee was struck in the head causing injury.
Employee entered through a man door right behind another employee and caught his left finger in the door. Employee did not seek medical treatment until 1/6/14 and was taken off work on 1/13/14 due to fracture to the finger resulting in surgery.
Employee was ascending the steps on the Cat 789C Rock truck when he slipped on the bumper, losing his footing and fell on the ground.
Employee climbed into a scoop and left knee locked up. Employee was sent for medical treatment and MRI which showed a bucket handle tear/lateral meniscus. His first date of loss is 12/13/13.
While lifting a pipe with a chain block a strap broke, which caused EE a strain in his shoulder. EE never reported it till 1/29/14. He went to the doctor on 1/31/14 and reported back to work as lite duty. We had no lite duty work so EE was sent home on 2/3/14 pending further evaluation. Contacted us on 2/20/14 - he will have surgery & will be off longer.
Individual was lifting trolley wire for initial installation on 13-Head gate section. When lifting the trolley wire overhead he felt a pop in the right shoulder.
Diagnosis from the DR. said to be an Acute reaction from lack of vitamins, sleep, and dehydration caused his body to go into convulsions. the employee apparently was going through alcohol withdrawals while he was sustaining his attempt for sobriety .
Employee stated that he was going to the loader when he stepped onto uneven ground. As he was climbing onto the loader, he felt a pop in his right knee. His knee started hurting and swelling later. Continued to work but went to ER to get it checked later that evening. Missed the next work shift.
EE was walking down No.& Entry when he ran into a torque tension roof bolt with his head. Felt pain in his neck and back.
Injured employee was operating company #67 man trip coming off the 8 Right section when the trolley pole came off the wire and broke. The harp portion of the trolley pole swung and struck the injured employee's left shoulder and facial area causing a bruise.
Employee was descending the steps on the Cat 789C Rock truck when he slipped on the bumper, losing his footing and fell on the ground.
Crew was pulling disco pipe into beltline with loader. Connection anchoring pipe to loader broke and struck injured.
Employee was installing a new seat, in a CAT Enddump cab. In the process of pulling the seat he lost his grip and fell back into the cab firewall with his left shoulder.
The employee was walking and tripped and fell extending his left arm to attempt to catch himself and resulting in a strain and sprain of his left shoulder. Employee began missing work on May 22, 2014

While employee was trimming stone a piece of debris entered his left eye under his glasses.
Miner stepped off Tower 23 concrete slab onto uneven ground, when he fell and twisted his left knee and over extended his right foot. Miner fell on same level.
Was walking from shop area to the fuel truck to start it up, area was icy causing the employee to fall onto his back and injuring his left arm.
While doing a team lift with 3 others on an awkwardly sized piece of lab equipment the employee felt a pop in his right knee as they lowered the piece of equipment to the ground after lifting it off of the counter. EE received Rx modified duty.
EE was given the task to install a new pump. When he was searching for a bucket to dispose of broken grout he ducked his head to go under 2 low hanging rubber hoses. He raised his head, it contacted a stem of a gate valve resulting in him jarring his neck. He evaluated by a Physician and taken off of work for his next scheduled shift. Returned on 12-13-13 full duty.
Employee was walking in the roadway on J-0 section when right knee popped.
EE pulled a pump off of a block and slipped on the ice and fell and hurt his back.
While removing an isolation shutter from a spare cubicle an arc flash occurred injuring a plant electrician.
Employee alleges that he was walking through flypads and was struck by a shuttle car. Cliffs Logan County Coal disputes that EE sustained an accident, as that term is defined in 30 CFR 50.2(h), or that he sustained an occupational injury as that term is defined in 30 CFR 50.2(e) or an occupational illness as that term is defined in 30 CFR 50.2(g).
Employee was pulling a timber across the track when he felt a pull in his groin.
The man was drilling a bolt hole into the roof near the rib to install a roof bolt. While lowering the drill head, a corner of the drill head struck the man on the upper leg.
Employee was walking from the time shack to the field office when he slipped on some ice and fell on his left side. Employee fell on his left arm and left chest area causing him pain.
At end of shift, in the process of beginning to fuel equipment, employee slipped and fell approximately 10 feet to the ground.
Bolter opp. was tramming the bolter from the deck and was trying to back around the corner of 5 L.T. He backed the bolter into the rib and brock the canopy jack that held the canopy up. When the jack broke the canopy fell down striking the employee in the shoulders and back.
Mechanic was trying to secure a tarp on an excavator. The wind was whipping and he lost his balance and fell on the ground. He landed on a previously injured knee (right). It was reported to the company immediately. Safety staff picked him up to be seen by a doctor. It was diagnosed as a fractured patella. He will see a specialist on Thursday at 9:15 a.m.
EE was checking the hose connection for location PP113. EE stepped onto a five inch diameter bulk hose and twisted his ankle.
Employee slipped on ice in the parking lot and fell on elbows and back - Received a fracture to right clavicle bone.
Employee was walking to the shipping department when he slipped on the ice and broke his ankle and fractured his fibula.
Ee slipped and fell on the ice. He was seen at the ER and diagnosed with a contusion to the tailbone. Site investigation and investigation by workers' compensation determined this was a work related injury on 12/31/2013, making this a reportable for lost time.
Employee started to get out of supply motor and put his right elbow on top of canopy and his left hand in handle and pulled himself up and felt his right shoulder pop. Employee later required surgery and started missing 03-14-14.
While lifting box of waterline out of mud hole he felt pain in lower back.
EE was trimming a piece of belt off the bottom of a door when he was finishing the cut. The knife came through the belt and into his left arm causing the injury. EE will return to work when the results of a drug screen are available.
Improper lifting of pipe, which employee believed caused hernia. Employee declined to provide information to the company.
WAS INSTALLING A BRIDGE IN THE F-25 SECTION WHEN HE CAUGHT HIS RIGHT PINKY FINGER BETWEEN THE BRIDGE AND THE COAL RIB AND FRACTURED THE FINGER.
Walking on metal stairs on top of silo. Slipped on an icy spot and sprained ankle.
Employee was bolting on 8A section. He was backing his drill steel out of the hole when the pusher came apart from the finisher. The pusher steel came around and struck the employee on the right side of his face.
The tire tech helper was guiding the 15 ton jack hanging from chain under a R190 Euclid when the boom got stuck on the frame of the truck and broke loose. Causing the jack to pop up and hit the tire tech helper in the forehead and nose.
Employee stated that he was loading crib blocks on the service scoop and as he turned to placed one of the blocks onto the scoop he tripped over a rock and fell to the mine floor injuring his back and right shoulder.
Employee was walking past a locomotive when he stumbled on a piece of coal causing him to lose his balance and strike his left bicep on the motor tearing it and requiring surgery. The accident didn't involve compliance regulations. The employee was trained and had a positive attitude. The injury was not related to clothing or PPE.
I am filing this report because employee called me on the 18th of December stating he had to have emergency surgery on the 17th of December stating that he thought he had come down with the flu on the 8th of December and had to take off work until the 11th of Dec & returned back to work still feeling ill & worked 4 days until they found a hernia in his stomach.
Emp was fueling dozer. Fuel cap was frozen shut. Emp pulled on fuel cap and fell backwards to ground.
EMP AT MAKE DOWN OPERATION PICKING UP 50LB BAG WHEN HIS FOOT SLIPPED AND HIS BACK WENT OUT RESULTING IN PAIN IN LOWER BACK.
At the beginning of shift while performing a walk-around inspection on a pit bus; a technician walked from the front of the bus to the driver's side and tripped over a rock in the dark which caused EE to fall to the ground. EE's left forearm was fractured.
Electric shovel was loading haul truck operator was working in when the shovel dipper (full) dropped, impacting the truck. This caused equipment damage and injury to operator.
An employee was walking around the front of a pickup when he slipped on ice and fell backwards. He landed on his back, left shoulder, and left elbow. Employee was taken to ER for physician evaluation.
Employee attempted to lift latch on tanker truck. Latch was rusted and partially frozen shut. Employee felt a pull in groin and left arm near elbow joint.
EMPLOYEE LOADING TRUCK WITH FORKLIFT. TRUCK PULLED AWAY FROM LOADING DOCK UNEXPECTEDLY. FORKLIFT FELL FROM LOADING DOCK TO GROUND. EMPLOYEE HAS BROKEN COLLAR BONE AND RIBS.
EE was scraping the top of the grizzly with the bucket of a loader when one of the grates became dislodged. The individual tried to lift the grate with a strap and clean under the dislodged grate. The strap became untied causing the grate to fall and pin the ee left ankle between the grizzly frame and grate. The individuals left ankle was severely fractured.
While Employee lifted lid to the shuttle car cable reel the SC operator pulled forward causing the lid to fall smashing Employee's left ring finger. Employee was transported to Raleigh General x-rays were taken and no broken bones. Employee was scheduled for surgery at CAMC Memorial Division for skin grafts to repair the finger.
Employee was walking across supply car when he slipped and fell and felt pain in his back.
While riding as a passenger in a on-site service truck, the service fuel truck run through a pot hole, causing the miner to hit his head on top of roof.
Employee was pulling 6"" pump hose across uneven ground.
The shop welder was attempting to straighten a metal step on the car body of an excavator. He was using a large pry bar and a propane torch to try and straighten the bent area. As he was applying pressure to the bar it rolled causing him to fall to the ground, when he fell the pry bar he was working with struck his gloved hand against the floor.
Miner was helping clean snow and ice accumulation in and around a production plant. Slipped on ice and twisted right hip. This resulted in an exacerbation of sciatica. Miner continued to work and made first request for treatment on 12/14/2013.
Employee was lifting a 40# heater and strained his back.
The employee thought he put the scoop bucket on the ground, but it was setting on a big piece of coal. While he was loading the block in the scoop bucket the piece of coal burst out and scoop bucket hit the side of his right foot.
Employee stated while moving a pump, he felt a strain in his right upper arm. Employee was sent for treatment. Surgery was scheduled for 1/2/14. This is his first date of loss.
Employee was working on top of scoop, changing hydraulic pump. He stepped down off the scoop onto uneven bottom causing him to twist his right knee. Employee had surgery as a result of this injury on 3/3/2014 and is now off work.
Injured employee was pulling cable off of a shuttle car reel on the 8 Right section when he strained his left shoulder. Employee did not begin to miss work until 12/17/2013.
Employee was walking beside a de-energized ram car. Went through a clear curtain the ram car was parked under. The operator of the ram car did not realize he had not cleared the ram car, and energized the machine engaged the steering swung the bed and caught him between the rib and the bed fracturing his right femur.
On 12/5/2013 ee stated in writing that he strained his back pulling up on a spud bar. He did not want to see a doctor and finished his shift. On 1/16/2014 he seen a Doctor for a back pain. He was off by the Doctors orders for 5 days and came back to work on 1/21/2014 with restriction. He will be reevaluated by the Doctor on 1/24/2014
Employee stated that he picked up a bottom roller leaning against the coal rib. As he walked to his ride to load the roller the rib gave way and a section 72"" x 45"" x 12"" rolled off struck him in the buttocks and pushed him to the center of the entry where he fell on the belt roller he was carrying, causing a contusion to his left chest & armpit.
He was removing a guide by cutting it into manageable pieces and his hand got caught between two pieces of timber which cut his finger.
Employee was exiting a 262C skid steer, the employee stated that he had proper footing while stepping off the skid steer and felt his back pop. Employee then stated his lower back was hurting. Employee was taken to the hospital and treated for an acute lumbar strain. The employee was off for 5 days and returned to work on 12/09/2013.
Employee was pulling on tie off blocks on 5 D belt to splice the belt when he felt a pop in his right shoulder. The employee started missing 12/16/2013.
Ee was doing a post trip inspection of the track dozer. He slipped on an icy spot on the dump and fell, injuring his ankle. He sought treatment on 12/05/13 and was referred to an orthopedist. The orthopedist placed him off duty and referred him to a foot and ankle specialist.
Employee was removing steel hydraulic tubes when the injury occurred. He was using a 1/2"" hydraulic air gun. He was lifting and twisted the tubes over his head to clear from the work area. He strained his lower back and had to seek treatment. He missed the following day and returned to work on December 5, 2013.
While working in the BMX 3 East mains section, an employee was spraying water on a rock pile to minimize dust when a piece of rock came off of the miner head and struck him on the right cheek causing a laceration.
Employee was using a cordless saw to cut a fitting and caught left little finger in the saw. Sutures required.
EE was positioning a gear for the continuous miner ripper when he felt a sharp pain in his lower back area and a burning sensation in his left leg.
On 12-3-13 employee stated he had back pain due to operating a front end loader, feeding raw coal to the prep plant, which was causing back pain over a period of time.
Employee was walking across from #7 to #8 entry on #3 unit when he felt his lower back pop. He had been loading pins and boards on the right bolter earlier that morning. This accident happened on 12/3/2013 he continued to work till he had back surgery on 07/15/2014.
He was shoveling at the headgate drive area on the longwall face. He stood up and cut his right ear on a roof strap. The result was 7 sutures.
The employee stated that he felt something hit the corner of his eye as he was installing a roof bolt. He removed his glasses to wipe his eye and felt a burning sensation in his eye and then flushed his eye but it got worse and he decided to go to the Dr. to have it examined the Dr. prescribed him antibotics to prevent infection and placed the employee off work for two days.
The employee was throwing over miner cable, when he stepped on a chunk of coal measuring 6"" x 2"" thick, with the outside of his foot. His foot slipped /rolled off the coal causing pain in the front left side of his foot. The IE was transported to the hospital and determined that there was a fracture of the 5th metatarsal.
Injured employee sustained a strained back while lifting a block building a stopping. Employee did not begin to miss work until 12/19/2013.
Employee fell and slipped off a ladder while taking pictures of the screen media on the secondary side. When she fell down she hit the screen structure and bruised her back, elbow, and wrist when she fell.
EE was in the process of tightening a take-up on the surface sampler belt using a wrench. As he was pulling on the wrench he felt pain in his left shoulder. EE continued to work up until 6-17-2014 when he had surgery to repair his shoulder. His first day of work missed due to this incident was 6-17-2014.
Employee was attempting to raise the flip up shield on the continuous miner and as he was lifting the shield his foot slipped causing him to let go of the shield in order to catch himself. This resulted in the shield coming down and mashing the employer's hand. The employee received a crack to the 3rd metacarpal bone of the hand.
Employee was operating the continuous miner and stated that he took the 1st cut on the curtain side and then backed the C.M. up to trim top. After repositioning the C.M. employee began trimming top and walked up beside the C.M. The process of cutting top caused the rear of the C.M. to bounce and bump against the employee's right knee. This resulted in right knee effusion/strain.
Employee was pulling on a diamond wire to free it after it had become frozen over night. During the process felt pain in lower abdomen. Visit to Doctor confirmed that it was a hernia.
Employee reported he strained his back while climbing/ascending ladder into operator's cab of Komatsu PC-650 Excavator.
Employee slipped on loose material that he cleared away prior to beginning work on a bindicator and strained left knee.
Employee was operating a locomotive when a collision occurred with the inby locomotive causing the operator to hit his left side in the operators compartment causing bruising to his left side. Employee went to the doctor the next day and was taken off work.
Employee strained left calve muscle picking up screen cloth.
Employee was riding in mantrip #8 in the 10WHG travel road. The mantrip struck a pothole causing the employee to feel pain in lower back. Lost time injury.
The employee was working indoors and running a splitter machine. He got his hand caught between the stone and the table.
Employee was walking behind Kiln #1 shale feed conveyor when he stepped in a rut twisting his right ankle. He was taken to the emergency room for evaluation where x-rays revealed a right ankle fracture.
Employee twisted his body around to retrieve a tool from his tool belt when he states he felt pain in the back. The employee received days away from work.
Claimant was walking from the scale house to the screen plant and slipped on ice and fell. When claimant fell, his ankle buckled.
Employee slipped on snow covered ice and sprained left knee.
The employee, was moving a suction line for the de-watering pump in the rain. The employee lost his footing due to the wet conditions. The employee strained his back.
Strain to the right shoulder occurred while holding on to the AMFO tote's bottom liner outlet in an attempt to stop the leakage of AMFO while transferring from large to small tote bag.

Employee was impacting bolts on a pipe flange with a 3/4"" air impact wrench when a piece of rust fell measuring about 6-8"" fell off the bottom of a belly pan and struck him on his left index finger about the knuckle.
Haul truck was being loaded at electric shovel. Shovel hit dovetail of truck while swinging into bed. Ee jarred in cab, c/o of numbness to right arm and hand, numbness and pain in right shoulder. Initially diagnosed with strain, released to full duty. MRI reviewed on 02/13/14 showed labrum tear treated with injection and physical therapy. Ee continued on full duty.
Employee jumped from a Grove crane as it overturned. Employee was attempting to move water pipes and a pump. The injured employee sustained torn ligaments in his hip and 3 broken ribs. Investigation is on-going.
Employees was doing a PM in a MCC and cleaning. While brushing out a breaker enclosure that was bottom feed it caused an arc flash.
Employee felt a ""pop"" in his shoulder while using a banding tool to secure product to a pallet in the warehouse.
Employee was working on Door400, the 400 ton hopper Mega door. While working on the door, the fabric lifting strap that raises and lowers the door broke and the door fell 8-10' down and struck employee on the head knocking his hard hat off. The door then struck his left shoulder and forearm that was on the railing of the scissor lift.
Employee was maneuvering water hose behind the heater in the corner of MD1 area. The nozzle of the water hose stuck open and employee had to retreat from area with water flow still on. Her hard hat got caught by a rubber gas line hose which caused her to start falling but she caught herself. After a few hours, her knees began to hurt. She was diagnosed with knee strain.
While the employee was conducting construction quality assurance for a project at the mine, he walked forward a few feet down the slope and stepped onto articulated concrete block mat (which had some gravel on the surface) and he slipped falling backward and landing on his left shoulder, causing a contusion and ruptured rotator cuff.
They were putting belt on conveyor. Chain came off of bar holding belt. Belt swung over and hit employee who was on a ladder and tipped him and the ladder over. Broke his left arm
Employee placed left hand on machine to brace himself while walking around equipment on the right side of engine compartment. He did not see the broken piece of mirror under the gasket laying on the cowl of the cab. When employee placed hand on gasket, the piece of mirror punctured his left hand between his two middle fingers.
Employee was putting a piece of angle iron 1-1/4"" x 1-1/4"" x 1/4"" back in place in the steel rack when it became stuck while sliding it in place. He felt a sharp pain in his right shoulder. The angle iron was about 12' long and weighed about 12 lbs.
Ind was getting off his buggy in coal yard to get supplies when he fell on snow covered ice injuring his lower back.

Employee exiting CR-112 pulverizer when his retrieval line got snagged. He bent down to unhook it, when he stood back up to exit, he felt a pain in his lower back.
EE states he was pulling a hose down the east side of 3 conveyor walkway on grinding line 11. While doing this, he felt a strain in his back. He was evaluated by a Physician and taken off of work on 12-16-13.
Parking area had material added to it in the a.m. raising the truck level so that final step to ground was about 3'.
While helping to change screen panels, felt something pull/strain in the lower right groin area. Since then, a bulge in the lower right groin area has appeared. Went to Doctor on 12/18/13 and was diagnosed as a Femoral Hernia.
Employee was cutting welds to remove a cover plate to access the inside of a conveyor receiving box. Employee was using a grinder with a cutting wheel when the grinder kicked back striking and knocking off his hard hat and attached face shield. The wheel then struck the employees cheek causing a laceration from the edge of his lip to his cheek approximately 2.5 inches in length.
Employee lost her grip on a hose and it struck her in the face. She was seen by a doctor, diagnosed with a concussion, and taken off work.
Employee stepped down a step with poor foot placement and rolled his ankle. Initially he thought he just sprained his ankle. The next morning (12/11/2013) the pain had increased and we sought medical attention for this employee. Diagnosed as a spiral fracture to the fibula.
Employee strained his shoulder pulling on a miner cable. He started missing on 2/14/14 to have shoulder surgery.

The employee was carrying a hoe when she tripped over a block of wood frozen to the ground. She fell and landed on the handle of the hoe. The breath was knocked out of her and she became very tender and sore on her right side near her ribcage.

Miner was dismounting ladder on 980 G FEL. Right knee buckled when miner placed right foot on ground. Miner was on flat clean surface.
Employee stated that he may have pulled a muscle in his right side while packing belt structure to scatter for the next set-up. He stated while packing the structure he felt a sharp pain in his right side that made him uncomfortable to stand, sit, or lay and the pain was getting worse. Employee later required hernia surgery and started missing 01-15-14.
Injured employee had been using a sinking hammer to drill holes for blasting of an overcast location. After drilling multiple holes his hand began to swell. It is believed that this swelling was caused by overexertion. Medical provider held the employee out of service.
Employee was helping set water pump in pit sump he stepped out of way, felt something pull in lower back, stated he was ok. Continued to work through 1/6/14. Due to weather was told to stay home on 1/7/14. Employee called in on 1/7/14 said he was going to ER due to the incident that happened on 12/5/13. He received no medical treatment and no lost time till 1/7/14
(Previously reported as a FA on 12/05/13) Employee was re-routing a water jack discharge. While installing a 20"", 90 degree fitting, the employee twisted his knee. Injury became recordable due to surgery on 3/19/2014.
Employee was walking down from parking lot, stepped on uneven ground and fell to the pavement, hitting shoulder. EE was taken to the ER, and released with restrictions within the scope of work. EE sought additional medical treatment on 12/11/13, was released with stronger restrictions for 12/13/13. Currently ongoing.
As the employee stepped to the ground his ankle rolled, causing him to fall to the ground. The employee stated that he misjudged the distance from the cab to the floor and resulted in a sprained ankle. A follow up visit with an Orthopedic consult identified that a fracture had re-occurred. The ankle had been fractured in an off the job event about a month ago.
EE WAS INSTALLING AN EXHAUST ON TOP OF A ROAD GRADER, HE SLIPPED AND FELL BACKWARDS ON THE TIRE. THE DOCTORS FINDINGS IS A BACK CONTUSION AND SPASM OF MUSCLE.
EE was doing his cleanup work, shoveling material and picking up and stacking brick, while doing so he felt a sharp pain in his lower abdomen. The Doctor says he has a Hernia NOS. He is to see a General Surgeon.
While wearing his safety harness, employee was inspecting the feeder pan when he slipped on a small rock and fell (on the same level) on to his buttocks.
Employees were engaged in cleaning spilled material from the zero to six conveyor. Material was hand shoveled onto the belt and then the conveyor was jogged and the process was repeated. During the 4th. time the process was performed the employee slipped and his foot fell into a self cleaning tail pulley. The employees sustained a fracture to his left fibula.
Employee stated that he and the section mechanic were lifting the miner cable up to hang it across an intersection when he felt a sharp pain in his right shoulder. Employee continued to work until 12-18-2013 when he had surgery to the shoulder.

While descending a 4 ft step ladder with one foot on the first rung, and the other foot on the second rung, the employee's boot got hung up on the step causing him to loose his balance and fall to the grated flooring below. He was transported to the company Doctor, diagnosed with a fractured 8th rib on the right side, given prescription medication and work restriction.
After getting off work he experienced some strain to his left elbow area and feels it must have been from stacking pavers 4"" to 6"" - waited until the next day to let the supervisor know.
While moving bolting materials from a bucket liner to bolting machine and in a kneeling position, the individual felt a pop and immediate pain in his right knee. Employee had to push knee cap back into place. Shortly after incident the knee became warm to touch and swollen.

Employee was building a ventilation stopping. A piece of coal rib fell, striking him on the left leg, causing him to fall to the ground. This resulted in contusions and a sprain to his left ankle. He was placed on restricted duty by the medical provider.
A mechanic was changing out drill steel when the bit sub tightened the deck table swung and the employee's left foot was caught between the deck cable and mass brace.
The technician's shoulder was jarred when the dozer he was operating rolled onto its side. He returned to work continuing his normal duties until he underwent a procedure and returned on June 6, 2014 with restricted duty.
The employee was using a sledge hammer on the kiln 4 chat bin to loosen material from the side of it. One of the blows of the hammer caused a pain in his right wrist.

Employee taken to First Aid where ice pack was applied, OTC Aleve was given for pain. He was released back to work. On 1/2/14 he was still having pain. X-ray done on 1/3 showed an avulsion fracture at the base of his lower left leg.
While adjusting the air doors the Left hand was smashed between the adjustment arm and the header of air door.
Employee was on the elevator when pulling down saftey cage. Employee felt a pain in his lower right back halfway into the pull. Employee was placed on alternate duty by physician.

Plant maintenance mechanics were replacing an old 30 ft piece of suction pipe. When they picked the pipe up the corroded end broke off, the end of the pipe kicked up and hit one of the maintenance mechanics in the face/neck area causing a laceration.
Employee was up on #2 ball mill removing man head. Employee was using 20lb sledge to hit a wrench that was on a nut on the man head. While swinging sledge employee missed the wrench causing him to twist un-normally and tense up causing a muscle strain in employees lower back left side.
Rock was lodged in chute. Employee tried to wrap a Chain around the rock and the rock broke free and trapped his arm in the chute. Pry bar was used to free his arm. Rock put a hole in his arm. Required stitches.
Employee was lifting the side cutter for an excavator bucket and experienced a thoracic sprain.
The employee slipped on a sidewalk, striking his elbow. An initial medical examination showed no injuries and he was only given work restrictions for the remainder of the day. Months later, pain STARTED in his shoulder. An MRI in July 2014 showed AC degeneration and tendonitis in the right shoulder.

EE was pushing the plate over the top of the tool box while another ee was on the other end guiding it to the bed of the truck when the plate came down pinching his middle index finger near the palm of his left hand. EE was wearing his gloves at the time of the incident.
Employee tried to unlatch a loaded dumpster with all of the weight on the latch and pulled left shoulder.

Employee was helping replace a shaft on a gear box. They were trying to beat the opposite side with a hammer. While holding the opposite side the gear box, it slipped and hit his hand. A small peace of metal was stuck in his left hand.
A welder with (approximately 23 years experience) was removing 1"" roundstock out of a steel rack on the rougher floor. The piece of steel was approximately 20 feet long, weighing about 53 lbs. As he was pulling the steel off the rack, he felt a pain in his lower back.
EE was unplugging chute, when rodding bar slipped from grasp, pinching the end of the right ring finger between doorway of chute and rod bar.

Material backed up into the chute and was forced out of the sample port - lime fines blew around his safety glasses and into his eyes. Using a pry bar to assist installation of a boom extension cylinder pin on mechanical scaler felt pain in back
EE was pulling on the switch and felt pain in his shoulder chest area. Employee was 11 hours into his 12 hour shift. Outside temperature was -21 degrees below zero. Dr suggested 3 or 4 days of limited use of right arm or shoulder.
He had just finished moving the electricians utility truck from it's covered parking area so that he could have access to move a fan out of the shop with the forklift. He parked the pickup in an uncovered area. When he stepped out of the vehicle his foot slipped on the icy surface. In an attempt to catch himself he slammed his other foot into the ground hard enough to cause injury.
Skidsteer operator was picking up rods with the skidsteer and the skidsteer had rolled forward from the pressure of him lifting and when it broke free, some of the rods came off the forks with one striking right foot.
Removing cable from a ""J"" hook, the hook came unhooked, fell and hit employee in the mouth.
Employee was tasked to check the cyclones and read the tank level at the ore storage tank. When he got to the tank, he called to the control room to shut down the 1 & 2 storage pumps because the overflow box was leaking onto steps. He went to the top of the tank of perform duties. When he came back down, he slipped on the wet steps. He strained his left shoulder and right wrist.

Operator of the #7 water truck hit a pothole leaving the water stand causing a low back pain.
They were rolling a belt onto the Cobra Saw and he got his finger cut and broken.
An equipment operator slipped after putting his chock block on a loader. Employee dislocated his right shoulder. No days missed, Restricted work only.
EE was assisting in installing an explosive box, as the forklift guided the box down EE got left middle finger pinched between box and truck. Laceration to left middle finger requiring stitches. Restricted duty until x-ray results are known.
The employee was using a hammer to bust a steel band on a pallet of blocks. The band snapped back and cut his knuckle causing a laceration.
Injured Employee (IE) was starting the Cemex unloading system and saw that the system was dusting. He went around to the Cemex Dust Collector while it was still running and opened the inspection port and stuck his right hand inside and contacted with a rotary feeder catching the tips of his two fingers on the right hand and pinching tissue on the ends.
988H loader operator was in the process of removing the wheel chock from the holder mounted on the loader when he felt a pain in his left shoulder. He felt the soreness would go away so he refused medical attention. On 12/9/2013, he indicated that his condition wasn't improving and he wanted to see the doctor. The doctor determined that he has a muscle sprain in his shoulder.
Miner was exiting a screen area. When he repositioned his hands to push himself up he struck his left elbow against a support beam that was behind him.
On December 10, 2013 an employee returned to work from his days off. He indicated that he believed he had injured his foot on 12/03/2013. X-rays on the 10th indicated there was a fracture in his foot. The alleged injury occurred when jumping down from a power center to the concrete floor in a surface storage area. The distance was approximately 3 feet.
The employee was adjusting impeller on a pump. He rolled the pump over by hand and his glove got caught in between the pully and the belt that drives the pump. He fractured one finger.
EMPLOYEE WAS SWINGING HAMMER, FELT STRAIN IN BACK.
EE & co-worker was installing a new head shaft in the #1 buggy in the last open break of the power entry. While splining of the head shaft, the item fell out of place and dropped approximately 2 1/2 feet striking EE in the top portion of his left foot.
The employee was hand loading roof bolt glue on a twin head roof bolter and twisted his back....
Employee felt pain in lower back in December 2013, did not report until March 2014. Medical evaluation on 4/3/2014 placed the employee on restricted work activities.
Miner was repairing CAT D-8R dozer at customer mine site. Jack stand slipped. Caused contusion to left index finger. Miner went to clinic for evaluation. Diagnosis was contusion. Miner was given splint to protect finger while working. Instructed to take OTC meds for pain. Returned to regular duty 12/10/2013.
Working in very muddy conditions and as employee was walking across drill pattern, he slipped straining his lower back.

Operating tractor pulling tandem pans. Excavating and hauling soil. Operator fell asleep returning from dump area. Tractor veered off road and hit cable crossing pole. Damage to front of tractor, no damage to pole or cable. Area was well lighted. Operator claimed no injury. Drug test negative. EE called safety on 12/4/13 & claimed lower back ache. Taken for exam same day.
Mechanic was working on a Ballformer. While one mechanic was lying on the ground making adjustments to the roller bearing bolts, the injured mechanic was using a track bar to leverage the support roller bearing into the proper position. The track bar slipped off the roller bearing striking the mechanic on the ground just above his right eye causing a laceration.
Employee was removing a conveyor when he struck his left shoulder on the side of it.
Employee was hooking up a 2"" hose to a pump. The clamp on the hose had a sharp edge and as he grabbed the hose to attach it to the pump, he cut his middle finger. He received 8 sutures to close.
Employee attempting to bore a hole with a cordless drill when the drill bit broke causing employee to lurch forward at which point he cut the thumb of his free hand with the portion of drill bit still remaining in the drill.

Employee was assisting another employee with an automatic dumping refuse dumpster. It triggered unexpectedly, recoiled, and crushed his left index finger.

Injured employee was struck in the mouth by continuous miner cable while attempting to hang the cable causing a chipped tooth. He returned to work his next regularly scheduled shift. Medical treatment was administered in the form of capping a tooth on 1/13/14. This incident is being reported as a medical, no lost time.
Employee was tightening a bolt on a vibrator when his wrench slipped and hit him in the mouth breaking his false teeth.
THE EMPLOYEE WAS BREAKING DOWN SOME WOODEN PALLETS WHEN A PIECE OF WOOD HIT HIM ON HIS NOSE. HE WAS WEARING SAFETY GLASSES WHICH PREVENTED THE INJURY FROM BEING WORSE.
Employee was cutting Q-decking (sheet metal) with a Metabo cutting tool and cutting disc. The tool ""kicked-back"" and the employee lost his grip. The cutting disc contacted his left index finger causing a 1 inch laceration. The employee was treated at the Morenci Urgent Care facility, received 6 sutures and released to return to work without restrictions.
Ee was using a knife to strip cable for installation when the knife slipped, cutting the left thumb. Stitches were required to close the laceration. Released to full duty.
Miner was removing a cap to free the journal from the anchor box. The anchor box was hanging from a cherry picker attached to the forks of a lift truck. The forks suddenly and unexpectedly dropped and cherry picker fell on the miner's hand pinching it between the journal and the cherry picker.
Employee caught his right middle finger between the basket of the JLG Lift that he was operating and a conveyor frame, fracturing the tip of the finger.

Knocking a piece of metal off of a sub in the yard with a pry bar. The pry bar slipped mashing his left pinky finger between the pry bar and the sub. This resulted in a laceration to the pinky that required 7 stitches to repair.
EE was standing next to panline with his hand resting on top of the spill board. The shearer went by & his hand was pinched between the bretby & the metal spill board.Cause of this injury was employee not paying attention to hand placement. Compliance with PPE rules/regulations/mining equipment systems not a factor. Miner proficency/attitude could have been a factor.
EE was throwing the flap at the top of the west elevator and his feet slipped on the wet Lime, plus it was raining. When his feet slipped he fell and hit his forehead on the edge of the chute. Resulted in a 2 inch cut that required ""Liquid sutures""(glue).
Employee was sampling flowable fill concrete at the Affinity Mine. Sometime during his shift he was splashed on the chin by two specks of wet concrete and did not notice until he got home that night. On 12/30 he sought medical attention and was diagnosed with two small second degree chemical burns on his face and prescribed oral antibiotics and an antibiotic cream.
Miner was a passenger in a truck. Truck was parked, parking brake set, and chocked. Miner was not seat belted in as truck was parked. While vehicle was dumping, it tipped over onto the left side. Miner fell approx. 3' injuring elbow.
EE was welding and he flipped his hood up to check the weld when something airborne got into his eye.
EE had finger pinched between hydraulic ram and stairway tread while moving ram down stairway resulting in sutures.
Employee was riding on the passenger side of a rubber tired man trip, when they rode over a 2"" discharge line causing the end to flip up and strike him in the face. It caused a laceration to his lip.
EE was trying to rotate a plugged pin-mill feeder with a pipe wrench on the shaft when it broke free, causing his left middle finger to get caught between the wrench and feeder. It required 3 stitches to close the laceration and there was no lost time from work.
He was taking down miner cable out of the hooks. As he was taking the cable out of one hanger, the hanger behind him came apart and the miner cable fell out of the hooks. The cable knocked him down and his hard hat fell off. He struck his head on the shuttle car and received a laceration to the right side of his head. The result of the laceration was four sutures.
Injured Employee exited shuttle car, slipping and hitting his chin on the frame of the shuttle car. The employee received 5 stitches to the chin. Compliance with rules and regulations was not a factor. Miners proficiency or job skills may have been a factor.
While changing bags in a cooler baghouse, an employee was stuck in the nose by a bag cage. This resulted in a laceration to his nose which required stitches.
IE was riding in a mantrip traveling to the bottom at the end of his shift. An unknown object struck him in the right shin. He received a laceration to his right shin requiring 8 sutures to close the wound.
THE EMPLOYEE, TRANSIT DISPATCHER FROM SCALE, WAS WALKING TO HER CAR AND TRIPPED ON A WATER HOSE, AND FELL TO GROUND. ACCIDENT RESULTED IN A FRACTURED RIGHT ELBOW AND LEFT KNEE CONTUSION. RETURNED TO WORK THE NEXT SHIFT.
Carpenter was stripping forms and bumped his head on a support member.
An Operator reached into bolt bin, when they felt an insect. They jerked their arm back and forth and thought they had dislodged the insect. A couple of minutes later, they felt a bite on their lower abdomen. They did not report the incident until several days later when it was evident that there was a possible infection with the bite location.
Employee was swapping over engine and radiator and was removing old hose from lower cooling line on trk467 793D haul truck. He was using a pocket knife and cutting away from himself and had both hands pushing one on top of the other for better leverage and the knife slipped out from under hose and cut right little finger.

Employee was walking behind the mine wet plant outside when he stepped into a frozen puddle which was covered with water. This in turn caused employee to slip hitting his head on the ice. He had a tooth that was cracked due to fall. He did not seek medical attention until Feb 24, 2014 and the dentist removed the tooth.
EE received a laceration to the right thumb when it was caught between a railroad tie and a pipe support. A coworker mishandled the railroad tie and dropped it onto the employees hand as he was setting it into place.
The EE was instructed to push in the 3I surge pile. the EE exited his loader to remove cones that were in front of the syntron hole. when the EE went to move the cones he slipped on ice/snow that was in the yard catching himself with his left shoulder.
Operator was working to unplug 273 feed bin. He was working on the bin for over an hour. When material started to flow, he used a sledge hammer to close the hammer gate. He tapped the gate side to side to close the gate. He lost his footing and caught his fingers between the hammer gate and sledge hammer handle. He sustained a broken left index finger and stitches.
EE was bolting on 002 section in 6 left heading when a piece of draw rock fell striking his canopy then fell from canopy striking EE on top right forearm. The injury required 9 stitches.
Employee was making a belt splice at 002 Section Head Drive when he cut his left hand with a utility knife while he was cutting belt line. He received 5 stitches.
Employee rushed to an Arc Flash fire with a fire extinguisher and received smoke inhalation
Employee was taking a barge hatch off using an air impact to remove the bolts. When the hatch was pried open with the bar it came flying up from the internal pressure and struck his hand. He was diagnosed with a bruised wrist.
Employee was using a sledgehammer to replace a metal plate on highwall miner. When he hit the plate it came loose striking him on the left side of face and forehead.
Employee was getting into an excavator and turned the wrong way and felt sharp pain in middle of low back. Treating with chiropractor.
Employee was operating Cat 990 Loader to clean around a railbed. As he was backing up and looking backward, he ran over uneven ground which jostled him. He claims to have heard a pop in his lower back and that pain followed. NOTE: Incident was first reported as a first-aid-only case on 10/24/13. Management was informed of a 12/18/13 visit to a physical therapist, on 12/19/2013.
EE was stepping off of the mantrip at #2 unit shack when he tripped over his jacket and hit his head on a roof bolt causing him to have 4 stitches.
Employee was loosening belt on roller for conveyor & had pain in right elbow & shoulder. He did not report incident until 1/3/14. He sought treatment on 1-26-14 & did consider care. He saw MD again on 2-19-14 & was given removable brace & referred to physical therapy. We did not receive these records until 3-6-14 - therefore we did not knew that was reportable until now.
Employee was attempting to close the sliding door of dumpster that was stuck. He asserted force in attempt to shut the door, it closed rapidly catching his left index finger between door and frame of dumpster.
The employee stated that he was helping to construct a stopping when he struck his right index finger with a hammer. The finger was splinted as a result of the incident.
Worker's hard hat fell off - when he retrieved the hat he stood up and hit head on a metal beam. He went to walk in center and required two stitches to the head. He was off the job for app. one hour and returned to finish his shift and reported for work the next day.
