On 10/31/2013 at approximately 13:25 an unplanned roof fall occurred at cross cut #106 on the Main West, entry #4, beltline. The primary means of support in the area was 6' point anchor bolts, and the supplemental means of support was 10' point anchor bolts. The fall was approximately 12' high. The openings approaching the fall are approximately 18' wide.

A roof fall occurred at this mine at approximately 8:45 pm at crosscut 146 on the #4 belt. Please refer to citation #9011798.
PLC processor card failed on the production hoist taking it out of service.
A unplanned roof fall located in the intersection of the 1st North West Main. XC-154, Entry #7. It was approx 22' in length x 22' in width X greater than 8' in height. There was pump line and cable in this intersection.
Operator was using a Cat 320 Excavator to place trees on burn pile. While tramming from tree line to burn pile the excavator quick caught on fire. Fire watch noticed flames and assisted operator to evacuate the machine. There was no time to attempt extinguishing the flames with fire extinguisher on machine.
The Hoist Car at Crabapple slope was inoperable for a period of time. It was determined that the speed sensor was defective. The speed sensor was replaced.
An unintentional roof fall has occurred in #2 Section #3 Entry 4-A belt 25 break. The roof fall was approximately 25' x 20' x 6'. There were no injuries reported and no equipment involved.
The slope hoist was removed from service due to a coupling needing replaced. Once repaired, the hoist was placed back into operation.
An unplanned fall of the mine roof at or above the anchorage zone occurred on the 1 North/8 Right section at 1 block of 8 Right crosscut #1 to #2. The fall measured approximately 12' wide by 40' long.
Hoist went into manual and could not be operated. Processor card lost program memory.
There was a unintentional roof fall that occurred on #2 units 3rd belt at crosscut #8, (width 25'X 25' 10' height, in intersection) We are not planning on cleaning this fall up. This area was supported with 72"" tension rebar roof bolts.
An roof fall was discovered at approx. 10:30 a.m. in the #2 entry on the 3 E.mains at break #'s 44 and 45. All approaches were timbered off. The fall did not interrupt ventilation and will not be cleaned up. The fall was not on the active workings. The fall was approx. 70' l. x 20' w. It was above anchorage but could not be determined how high it fell out in intersection and entry.
Monitor Screen locked up. Hoist would still operate. ABB dialed in remotely and determined that spare PC installed on Oct 20th had too many programs running. ABB Tech Support suggested we switch our built in spare computer and the PA1 Computer. Swapped computers around. ABB did remote testing & released the Hoist at 3pm.
An unintentional roof fall was found by mine examiner in the intake just outby spad 679.This fall was immediately called into MSHA because primary escape way blocked. No ventilation blocked. No injuries. Approximate dimensions 12x12x6.
United Taconite plant lost power due to a polarity issue in a transformer. As a result our overhead crane operator was stranded in the crane until power was restored. The crane is installed with an emergency evacuation descending unit but it was determined that this situation did not require him to use it.
The elevator in the #9 intake shaft became inoperable due to malfunctioning of the door.
A roof fall occurred in the #6 entry of Main A at survey station 17. The fall was approximately 30 feet long x 20 feet wide x 10 feet high.
An unintentional roof fall was found by mine examiner in the intersection of Spad 1399 in #7 Entry of Old 4 Right Mains. This fall was immediately called into MSHA because travelway blocked. No ventilation blocked. No injuries. Approximate dimensions 18x18x8. Mine in Non-Producing Status since 8/13/2013.
An unintentional roof fall was found by mine examiner in the intersection of Spad 4346 of old 2 West Mains panel. Approximate dimensions are 18w x 18L x 10h. No ventilation or travelway blocked. All approaches dangered off. Fall will not be cleaned up.
2 RG Johnson employees were entrapped on top of Sugar Run Elevator. They were using a rope and it got caught in hoist ropes, causing plank switch for governer to fault. Removed from elevator 11:28 pm.
A roof fall 16 feet long 16 feet wide above the bolt anchors in the number 9 entry at 84 crosscut in a 4 way intersection. Ventilation was not affected. Passage was affected but the number 9 entry is common with the number 10 entry which is the normally traveled entry.
At 2:20pm 2 EE's heard a fall in a bermed off notch of U63A H crosscut. GF investigated and reported at 2:50pm that it was at or above the anchor zone. Incident immediately reported to MSHA. Area bermed off. Investigation determined adjacent reliever had not been shot to desired depth thus causing high stress to the area.
Ignition happened at 11:45 pm. Employee stated that when he saw the ignition, he was bolting on the operator's side when it happened. After he handed to the fire extinguisher to another employee, he knocked the power. When it appeared to be out, he blew dust on it to make sure it didn't catch back up.
The continuous miner cut into a sealed area from 10 to panel 9.
An unintentional roof fall occurred in 4 West Mains area at 24 wall between #1-2 x-cut and in the #1 entry. The fall measured approx. 8ft high by 16ft wide by 60 ft long. There was no injuries or equipment damage as a result of this incident.
Witnesses stated,when walking down slope belt, the slope car cable was observed to have a damaged place. The slope car was not in use. It was immediately locked and tagged out. All personnel underground were notified of the situtation and the availability of the emergency hoist. MSHA and State officials were notified within 15 minutes. No persons were injured in any way.
Mine Foreman was conducting a weekly exam. He found a roof fall in the section return (entry no 1 )between spad 7+00 and 7+50. The large horse-back was at the anchorage of the 5 foot bolt. MSHA was called at 10:30 A.M. The area was dangered off, then cribbed, and the return air was rerouted to entry no 2.
Roof fall occurred in an outby track entry. The fall is 20 feet by 20 feet and 12 feet high.
An unintentional roof fall occured on the Main West Travelway, crosscut 25, entry 3. The fall measured approximately 25 feet in length, 18 feet in width, and 11 feet in height.
In the Upper Fence Decline drift at the Pete Bajo Mine a ground fall occurred. The failure occurred as the drift was broken through to re-connect to an existing drift.
An unplanned roof fall above anchorage was discovered during the Preshift Exam at Survey Station 7893, approximately 1200' outby the 2nd Left working section. The fall measured approximately 20'L x 20'W x 9'H and has been dangered off from all approaches.
Main Computer PC in PA1 Cabinet failed. No personnel were at risk. Changed computer to Maintenance computer.
The Hitachi Excavator (EX1100) Operator observed a fire in the engine compartment. The operator climbed out of the vehicle, maintaining a safe distance from the fire. A 9-1-1 call was made and MSHA was notified by phone. The County and City Fire Engines arrived and extinguished the fire within approximately fifty minutes from when it started.
A roof fall occurred on White oak empty side track at breaks #121 to #122. This fall is 30,035 feet from our nearest working face and does not interfere with our air course.
Elevator was down due to an electrical problem.
The elevator door did not close fully due to material build-up in the track; this prevented the door switch to make contact causing the doors to stay closed. The Cal. licensed elevator technician cycled the power allowing the doors to open so that the employee could walk out of the elevator; the technician then cleaned the track and switches. Contacted MSHA and they laughed.
An unintentional roof fall has occurred on the #1 Section Sycamore #1 spad # 3335 break #169. The fall was approximately 25ft x 18ft x 4ft - 6ft. No injuries were reported and no equipment was involved.
Slope hoist became inoperable due to power outage.
A roof fall above the anchorage zone of the installed roof bolts has occurred in the No. 7 return entry at the No.8 crosscut survey station spad no. 1533, along the No. 12 conveyor belt haulage way- on the No. 1 unit.
Roof fall occurred 4 crosscuts East off 4-7 fan shaft, due East of the 4-7 overcast line. The fall is approximately 80 ft. in length and 15 ft. wide. The roof fell out in a semi-circle shape along slickened surface leaving a 4-5 ft. brow along the length of the fall.
A roof fall was found at 8:50am in our S7-1st Right Panel at crosscut #12, entry #6, a primary escape way. The fall measured 18'x18'x6'. The MSHA Hotline was notified at 8:55am. This fall did not impede passage of miners, affect ventilation or injure any miner.
An unintentional roof fall has occurred on the #1 Section #5 Entry spad 10013. The fall was approximately 30ft x 18ft x 5ft. No injuries were reported and no equipment was involved.
At approximately 12:45am the slope break car became inoperable. Electrical problem was corrected and the hoist was back in operation at 3am.
An unintentional roof fall occurred on the 2nd North East Mains, #8 entry, At or near spad #615. The fall measured 80' x 20' x 8' to 10'. It appeared to be a horse back slicken side formation.
The slope hoist was removed from service to repair a motor coupling. The hoist was repaired and placed back into operation.
The main cage was being used to transport the belt winder spool. When the cage was about 10 ft off the bottom the load shifted, causing the cage to shift. This caused the automatic safety dogs to engage and abruptly stopped the hoist. This caused damage to both of the cage doors. The cage was operated manually and returned to the surface for repairs and investigation.
Roof fall occurred in Gold Mains intake #8 entry at survey spad 967. Fall was approx. 20'x20' in size. Fall did not affect ventilation and is not in an area normally traveled by men.
A non- Injury unplanned roof fall occurred between # 8 & #9 entry between the Main North belt and the secondary escapeway at spad 2+40.
Heat from a defective take-up roller bearing is believed to have caught the belt on fire causing approximately 2000 ft of belt structure and the #3 head house to be damaged beyond repair.
A roof fall was found between the #3 to #4 crosscut at the #6 Belt head near survey station 2744. The fall measured approximately 30' in length, 17' wide and 7' high. No equipment was involved and no injuries reported.
Unplanned coal dust ignition in the #2 Coal Ball Mill occurred. No property damage or injuries occurred.
Employee was looking for a busted water line when the fall was found. The fall was located in the return travelway 40 feet inby spad no. 1938. It was reported outside and MSHA was notified. The area was dangered off until inspected. The fall was aprox. 150' long 4-6 foot thick. 55ton jacks and wooden timbers were installed to protect miners and the area affected.
A roof fall was discovered in the return air course between the East Mains Seals and Return Shaft 1 break outby spad 211. The fall was located in an intersection and was approximately 50' long x 20' wide x 8' thick. The fall will be dangered off.
An unintentional roof fall occurred on the 5 West Long Wall, Crosscut 56, in the intersection of number 2 entry. The fall measured approximately 18 feet wide, by 18 feet long, by 16 feet high.
The elevator was run in manual mode and an e-stop was activated surging and braking a shut down bracket and shut down switches. Repairs were made by 7:05 am.
An unintentional roof fall occurred and was discovered by mine personnel. No injuries or equipment damage occurred. Fall area measured 6 feet highx 16 feet long x 16 feet wide. This fall area had previously been roof and rib bolted to plan , with roof screen installed. This fall material consisted of laminated shale with a clay seam parting , with water present.
Wana Elevator- Hoisting a scoop car on the service elevator to the surface when a steel choker rope broke hoisting the scoop allowed the scoop to fall 10' causing damage to the elevator floor.
Tooth on loader bucket came in contact with a primer which had failed to detonate at the time shot was fired. The primer detonated when struck by the bucket tooth struck same.
Employees using cutting torch to remove bolts from valves slag fell and traveled on foot and fell into a hole measuring 2 inches in diameter and contacted the packing, starting a fire.
A ROOF Fall occurred in the No. 3 to No. 4 crosscut of the 7-1 development section to the left of survey station No. 4091 in a return air course. The roof fall measured 50'L x 20'W x 5'H.
An unintentional roof fall has occurred the #2 Section Left Return 6 break on 6A Belt. The roof fall was approximately 30' x 16' x 5'. There were no injuries reported and no equipment involved.
An unintentional roof fall occurred in 4 South Mains area at 28.5 wall in the #6 entry. The fall measured approx. 8ft high by 40ft long by 20 ft High. There was no injuries or equipment damage as a result of this incident.
A roof fall had occurred on our #2 Main Belt at 8+83. The fall was 26 feet long, 17 feet wide and fell above the roof horizon of feet. Area was roof bolted with two piece roof bolts, headers and pie pans. Fall was caused by deterioration of the mine roof through weathering.
A high concentration of methane was found in our 3W-1st Left Seal Area. MSHA was notified at 9:30am and all miners were pulled from the mine. The investigation showed that our #8 De-gas well had been plugged for an unknown reason. A new well was drilled and the methane in the active areas of the mine was reduced to harmless levels.
A roof fall has occurred in a intersection of the right return of the Queens Fork mains it approximately 18'x 18'feet wide by 4'feet high
6 worn and 1 missing miner bit and two sprays not working properly contributed to a dust ignition that lasted for 2 or 3 seconds in the E-26 section.
An unintentional roof fall was discovered by a mine examiner at 47 crosscut in 2-1 entry in Cumberland West. The fall was in a crosscut, above the anchorage point of the roof bolts. No ventilation was affected.
A non-injury roof fall occurred in the #3 entry adjacent to the #2 entry #14 beltline just inby spad#2513 at break256. The fall partially blocked the #3 intake which was the main travel way to 001 section and is the main intake to 001 section, but didn't impede the ventilation. The fall was approx. 6'H. x 18' W. and 60' L. See attached map.
At 10:45pm the electricity providing power to the main slope hoist was disrupted. Fuse holder on incoming line was found in disrepair and replaced. Hoist was in operation at 1:15 am 10/8/2013.
Idle Mine A non injury roof fall was discovered at #6 break on #6 belt in the #2 entry in the intake. The affected area is to be cribbed off. The fall was approx. 20' x 24' and about 9' thick.
Overtravel switch became inoperative on the service hoist.
The Crabapple Elevator was not operational for an extended period of time. This was due to the brake solenoid fuse failing. The fuse was replaced.
Roof fall was discovered in the 7th North West Submains, XC-17, Entry#3, Spad 14+50. This was the intersection of a neutral entry, non-escape way. Approx size 30'in length x 30' in width x greater than 7' in height. There was a pump, barrel, discharge line, and power cable in that intersection.
A contractor trailer caught fire causing complete damage to trailer.
A roof fall was discovered in the right return a spad 1277. The fall was discovered on above date and approx time. The fall was found during the pre shift examination when low air velocity was found on the right side. Upon investigating the cause of the low air the fall was found by employee.
ROOF FALL: A roof fall occured on the No. 5 section above anchorage of the roofbolts that were installed. The roof fall was approx. 25 to 30 feet in length, 15 to 18 feet in width and 8 to 9 feet thick.
Elevator at #9 portal became inoperable at 12:01 a.m. due to a middle leg of stater for the elevator burnt up and 400 amp fuse blew. No men were on the elevator at the time of the incident. The elevator was operable at 3:50 p.m.
An unintentional roof fall has occurred on the #1 Section #2 Entry at Sycamore 2 head spad 5108. The roof fall was approximately 25ft x 20ft x 3ft. There were no injuries reported and no equipment was involved.
A roof fall was discovered on this date. The fall was located on the main line at break #19 in the #3 entry. The fall measured 7' high by 19' wide and was 60' in length. The area was initially supported with 5' grouted rods. The area was dangered off. There were no persons affected.
Old tires that were piled in an abandoned portion of the pit for disposal caught fire due to spontaneous combustion. Upon discovery, the pile was covered with dirt by track dozers that were called to the area. The fire was extinguished shortly after the 30 minute time frame.
Elevator at #9 portal became inoperable at 2:45pm due to the recent air change (air pressure changed). No men were on the elevator at the time of the incident. The elevator was operable art 9:30pm.
An unintentional roof fall occurred on Unit #3 in Entry 1 at the 52+80 survey station. The fall occurred due to the roof strata broke at the anchor point. The fall area had been supported with 4ft. and 5ft. conventional bolts. The fall is approximately 40 ft. long, 20-25ft. wide and 0-5ft. high. The fall will be cleaned up and re-supported.
EE had a fall of ground in the 5170-210 Drift. The heading was in the bolt cycle. Operator noticed ground movement then he pulled the bolter out to a safe location and heard the ground fall.
An unplanned fire occurred on the D11CD Dozer (company number 370) while operating on the L9 in the Remine pit.
Elevator at #9 portal became inoperable at 110:50 pm due to the starter mainline fuse. No men were on the elevator at the time of the incident. The elevator was operable at 3:15pm.
The miner was in progress of storing electrical cable into a cable tray. The south bracket some how came loose. When the south side of the cable tray came down it struck the ladder on the east side causing the ladder to fall towards the west. The miner tried to compensate for the fall by leaning to the west. He then fell from the ladder hitting the ground.
A shuttle car driver was tramming through a crosscut from Entry # 5 to Entry # 6. The car driver stopped spoke to the victim. The victim proceeded past the car, traveled through a curtain into the intersection of Entry # 6. The victim told the car driver he was clear. The car proceeded through the curtain and struck the victim between the shuttle car and the left rib.
Employee was building drill bench, pushed thru the berm at the edge of the highwall and traveled over the highwall.
The victim was operating a battery-powered, rubber tired, personnel carrier on the 2nd 48 supply road. As he traveled outby, for a presently undetermined reason, the golf cart veered suddenly to the left before overturning on top of him. Co-workers initiated CPR and continued to the surface. Attempts to revive him were unsuccessful and he was pronounced dead at 1:59pm.
While in the process of pulling top chain onto the longwall conveyor panline, the chain became fouled. While examining the area, an apparent failure of the sheave anchor point may have occurred allowing the rope and sheave to be propelled forward. An unknown portion of the assembly apparently struck the injured causing fatal injuries.
Worker's right hand was caught in a running machine which resulted in the loss of the tips of his 3 middle fingers and damage to his thumb. Worker did not follow proper lock out tag out procedure to ensure that the machine was locked out.
While installing roof bolts and hog wire to the mine roof, ee used his left hand to hold up the hog wire, the hog wire got wrapped around rotating steel, pulling his left arm around the steel.(causing the amputation of his left arm just below the elbow)
At or around 6:50 pm on 10/23/2013, a DuPont Operator was in the process of clearing an obstruction from the turn table with an alignment bar when the employee's thumb was pinched between the alignment bar and structure. The EE was transported to the hospital for treatment.
Employee was installing a 8' bolt. As he was adding a 4' extension steel on the initial steel, he dropped the pod and it came down on the steel he had resting on the boot of his right foot. This caused the steel to come thru the top of his boot, smashing the middle toe of his right foot. Toe was amputated on 10/22/13.
During stretch and flex activity EE over stretched and experienced a sharp pain in groin area.
While sliding a ""C"" channel to the front of the bolter, employee had his fingers in the slot of the channel, the channel slid over a crib block amputating the tip of his right hand ring finger.
Employee was spoting up for a stopping on the #1 section. While backing the scoop out of break, the door on the scoop opened. He attempted to shut the door while continuing to back the scoop up when his fingers were caught in between the door and coal rib.
EE was located beside dock ramp, when another employee pulled pin releasing dock to loading position on truck. EE reflexively extended arm as dock sprung up. The hinged end of dock plate extended up and pinched fingertip of employee. Tip of finger was removed. EE returned to work with restrictions after one lost workday.
Employee was lifting a piece of pipe and felt a tearing sensation in his abdominal area.
Was walking belt and bent over to examine the bottom belt. When he stood back up he ran into an 8"" water line that crossed the belt and fractured his nose.
Employee was using a slate bar to remove pins from the right angle conveyor as the pin was dislodged, the shifting weight of the conveyor caused the slate bar to pivot upwards, causing the slate bar to strike him on the right cheek.
Employee alleges hitting his knee on bucket lever of Loader.
Operator was climbing down the ladder of a 740 swivel truck when he lost his footing causing him to fall to the ground landing on his right arm and shoulder.
Employee tripped on 2X4 in his work area and landed on right knee.
Employee was in the process of installing permanent roof support as he leaned forward to straighten bolt he lost his balance causing him to fall forward, mashing the tip of his right index finger between the roof bolt plate and the boom of the bolter. He was taken to local ER and referred to a hand specialist. Surgery was needed to repair the damage to his finger.
Injury was not reported at the time of the incident or while on SWN property. Injured employee reported to his dispatcher after leaving the sand mine that he was getting down from his trailer after opening his lids and slipped on the trailer ladder causing him to fall to the ground. Driver was hooked to SRL at time of incident.
While sheaving a ram jack onto the Longwall drive motor, the jack got caught on a post. Employee approached the area and the jack kicked sideways and struck his right leg.
Employee stepped backwards off bottom rung of 3' stepladder injuring right knee. Event was not reported for 5 days and there were no witnesses. Medical evaluation was strain to knee. Report was given to Montana State fund and on 12/06/2013 Montana state fund determined it was a work related injury
Employee was hanging cable when he strained his back.
While hanging shuttle car cable draw rock fell from between bolts and hit EE on left leg and foot. Rock size 40"" wide 10' long and 6"" thick.
On 10/30/2013, an employee was descending steps in his work area when he heard a ""pop"" in his right knee. The employee was evaluated and received a full work release on the date of initial injury. On 11/08/2013, the employee was again evaluated for on-going symptoms of his right knee and was diagnosed with a partial tendon tear.
Employee was operating miner when the rib rolled off of the corner striking him on the right foot. Fracture to the right foot.
While changing a cutter motor on a longwall shearer, the employee's right knee was caught between the old motor lying on the toes of a shield and the new motor; when the tuggets suddenly shifted allowing the new motor to shift against his right knee resulting in a contusion.
Employee was stepping off stone and twisted ankle.
The employee was working in the #16 East Ridge Tunnel, a skirt from the belt frame fell off and struck the employee on his hard hat, causing pain in his neck and upper back, requiring physical therapy treatment.
While repairing a loader he slip and he hurt his left ankle.
Walking down gravel ramp lost footing and put his right arm out to catch himself.
Employee stated that he was removing the block from behind the tire on the rock-duster. As the block was removed the duster rolled back and caught his hand between the tire and the block. Employee states that he jerked his hand back and felt pain in his elbow. Medical received on 11/21/13 diagnosed the employee with an elbow strain and the incident became reportable.
Employee was attempting to remove a chunk of material caught between a chute and a conveyor with a pry bar. The bar slipped off the chunk contacting the belt this caused the bar to bounce back striking the employee behind his left ear causing a contusion and stiff neck.
Employee stated as he placed a pin board on a roof bolt he was installing, he felt sharp pain in his left shoulder.
While walking in the BMX 1A Tailgate section, employee stepped into a haul road rut and twisted his left ankle.
EE was loading a metal toolbox into a rail runner and when he lifted on the toolbox, he felt a sharp pain in his low back.
While laying out lay flat rock dust hose, the employee felt pain in his right elbow and arm. This incident became reportable on 10-31-2013 when the employee began missing work.
As the EE was stepping off of the service truck he twisted his L ankle on uneven ground.
Employee was carrying a bucket of miner bits when he felt a pull in his back. Employee tried stretching his back to get some comfort and when he did he felt a pop. He immediately came outside and was transported by EMS to Raleigh General Hospital for treatment.
Employee was splicing conveyor belt with proper splicing equipment. As the belt was being tightened with comealongs the clamp became wedged against a trough roll. It was not noticed that it was stuck. As the belt was pulled tighter it finally let loose and slid off the end of the clamping bar struck him in the right shin.
A rock lodged behind a hopper wall three feet above and to the right of the miner's right foot slid and struck the foot in the inseam behind his right toe. First aid for minor bleeding was given and the miner was driven to a Care Now Facility. It is not known at this time whether the injury will heal itself over time or will need to be surgically repaired.
Employee fell while exiting the man trip. His tool belt was caught on the steering wheel while he was stepping out. He fell backwards.
Employee was placing stone onto conveyor system for thin cut saw when the piece split into two pieces and one of pieces fell on top of his right foot between the steel toecap and the ankle. Employee was sent for X-rays, which were negative and told to return to work on Wednesday.
Pulling a rock dust hose to the track entry got feet tangled up in hose and tripped hit draw head on the track motor
Individual was walking over a pile of coal behind the miner when he twisted the right knee in the loose coal. Complained of pain and swelling in the right knee.
Several employees were attempting to unplug the #2 - 4th stage cleaner feed pump with air. The 8"" HDPE suction line came apart at one of the welds. An employee was standing near the front of the line that came apart when the material was ejected from the line onto the employee's torso.
EE said he was moving the kiln gun away from the kiln after shooting a ring out and slipped off diamond plate and hurt his pinky.
Employee was cutting the strut on miner with a torch - Flame caught pan jack hose on fire, burning his nose and right ear - Drove himself to hospital
EE slipped on frost covered rear loader tire falling onto the tire injuring right ribs and shoulder.
Employee was inside a chute below Screen 2 getting measurements for a replacement divider wall. While exiting the chute he struck his left knee on a ""sharp piece of metal"" which inflicted the injury.
EE was changing the trans filter on the 988H. He pulled the filter, turned to move it from it's position. He pulled a muscle in his back. EE was scheduled to be on vacation the rest of the week. He went for follow up yesterday. He had to bend backwards then forwards. His muscles knotted up again unable to work. His 1st official day out of work was yesterday. 11/4/13
Employee heard noise, as he was walking in surface area near highwall miner, he tripped on pile of coal & fell, hitting head on miner.
Employee was on top of a continuous miner adjusting the water sprays. As he dismounted, he slipped and landed on his left foot, twisting his knee. The accident became reportable on 3/12/14 when the employee started missing work.
Employee stated that he was walking across the #2 section when he stepped in a hole and twisted his knee. This resulted in a left knee sprain.
Employee was hoeing material out from under a conveyor belt. While pulling material with the hoe, he lifted his body from the waste and felt a pop in the lower right-side of his back.

He went to ER and had x-rays completed that showed negative findings of a fracture, he is off right now due to swelling and pain in right foot.
Employee was adjusting the gates on the screen deck of the crusher and he knelt on a rock. The knee became swollen.
Employee was crawling in low roof when a piece of stone struck him from the roof where the mat screen was separated.
Carrying a 5 gal. pail of refuse material claimed injury occurred on 10/25/13 did not report injury until 10/29/13 off work starting 10/30/13.
Crane operator (74 yrs old) fell 5+/-ft from a crane access step to the steel deck below. He was fueling and checking fluids on the Mantiwoc 4600 crane when he fell. He received fractured ribs, fractured collar bone, head & face trauma, temporarily unconscious and bruised lung. Was transported to local area hospital by ambulance.
EE was working in a van, roll-up door was open part way. Was climbing in. Part of body & one leg were in,& was pushing himself the rest of the way in. Foot slipped, & scraped his right shin on the edge. Continued working. That night, noticed a big bruise on his leg, and it was swollen. The next day, it was worse, so he went tot the ER. He had a strep infection in his skin.
While removing 100 lb bag of sand from a bagging machine, employee felt pain in his left shoulder and neck.
Miner was tossing 30 pound bags on the blast site when he felt a pain in left shoulder. Did not have any further pain so he did not report the incident until 11/5/2013 to Hilltop Energy. He said he awoke on 11/5/2013 with pain in shoulder went to doctor.
EMPLOYEE DISMOUNTED FROM THE CLARK FORKLIFT OPERATOR'S SEAT AND SUFFERED AN INJURY TO HIS LEFT TESTICLE. EMPLOYEE FELT SHARP PAIN IN LEFT TESTICLE.
Injured employee was hooking a tow strap to the Miner Cable to pull slack with the Ram Car when he felt a sharp pain in his lower back and groin area. The employee was handling a cable. The employee began to miss work on 11/26/2013.
Employee states that he slipped after stepping to the ground from #23207 16M Grader. Employee states he fell on his left side injuring his left shoulder, elbow and hip. Employee was examined by Dr. on 12/3/2013 and diagnosed with a strain/sprain of the left hip and lower lumbar. 12/3/2013 was the employee's first day away from work and is now a Lost Time Injury.
On 11-14-13 the employee requested medical treatment, was taken to the Company clinic and was placed on restricted duty which is being accommodated. The employee alleged that operating the excavator and possibly the bobcat caused pain in his lower back.
While hydroblasting the airlift on R-45 #80 precipitator tank, the employee went to top floor of the precipitator tanks to monitor progress of the #80 tank. The tank ""burped"" splashing the employee with effluent.
Picked up a box of glue for the roof bolter and strained his back the employee started missing work on 11/5/2013

Installing D11 Trans & Bevel gear using a prybar & strained lower back
EE was replacing bad bolts in the SB-920. He was using an air impact to tighten new bolts, while in the process of tightening a bolt an old bolt popped loose hitting him on his right ear. When the bolt hit him on the ear he lost his footing and tried to catch himself with his left arm. He said when he caught himself he twisted his left arm and wrist.
Employee was descending from D11R dozer (Company No. 394) when employee slipped and fell 3 feet onto the fender striking his right hip against the machine.
While shoveling & dusting the coal ribs employee stated that he noticed a red area on his left forearm.
While filling up the pod duster, employee was laying the hose back at the fill tank and felt pain in his back.

Employee was climbing up the steps on the haul truck, after heating his lunch, was not using the hand rail when his shoelace clasp on his left boot caught the bottom of his right pant leg. This caused him to trip and fall back between the bumper of the truck and cab, bruising his back. He applied ice, was taken to the ER and taken off work one day.
Roof bolting in 1st South #1 Entry, loose piece of rock fell between the ATRS and canopy striking the employee on the right arm.
He was walking into security building. A cat was in doorway, he tripped on cat, fell onto a chair and twisted left side and ribs and pain in neck.
While operating a scoop, employee felt strain in back/neck when his back was jarred while traveling over crib.
The employee was bolting on the miner and a piece of rock fell from the top striking him on the nose.
During a pause on the line, he leaned forward & lifted his left foot up & set it on a slow moving foot high conveyor belt. His foot moved along an inch or so & slipped down between that belt & the next one in the line, which was not operational. Upon removal he went to the ER-nothing broken just swelling & missed one day on ice at home. (No damage to steel toe boot either.)
EE tripped going up a set of steps leading into the changing room. EE fell and landed on his dinner bucket hitting his lower back.
Employee raising safety rack. The chain slipped and employee went to grab it and twisted back.
He was placing a vertical pin on a longwall shield. He asked another employee to activate the relay bar to drop the pin into the hole. When the relay bar moved, it caught his left index finger between the pin and the panline causing a laceration which required 21 sutures.
Material build up on the floor of Thickener tunnel. EE didn't realize how deep it was & waded thru it to reach a valve to unplug the sanded underflow lines. He was wearing his rubber boots at the time. Material was higher than his boots & spilled into them. He left the area & rinsed off his legs. He was taken to the clinic then transported to a medical facility for treatment.
During plant repairs, injured person removed a metal guard and set it aside. The guard tip over and hit above the metal toe on his right foot. This caused a break in the foot. Tried to keep working but foot began to swell with discomfort. Went to walk in clinic.
Hanging miner cable in hanger and ran out of slack. Employee strained back to force cable into hanger and felt pop in back.
Occupational illness. Employee scraped leg when rock came out of headbox. First aid treatment at home--no reporting. 5 days later, employee had infection in leg. Taken to hospital where he received IV therapy and hospitalization. First reported 10/24/2013. Still did not know if it was work related until 10/31/2013
While climbing into the back of a mantrip he twisted his knee.
Contracted worker was exiting the mine on a bus driven by mine personnel escort when it collided with a motor hauling pipe traveling into the mine.
Employee was driving refuse truck on refuse pile. The truck hit a bump on the pile and jarred lower back.

Employee was installing belt hangers when his drill steel became stuck. The employee tried to spin the steel out, while placing his hand on the drill steel, his hand got caught, wrapping his arm around the steel causing a compound fracture to his right forearm.
EE was walking through tool box storage area when he tripped on the corner of a tool box and fell into another tool box contacting his face. The employee received a fracture to the orbital area of the left side of the face.
Employee was on the longwall face where the shearer was cleaning up the tailgate area. As the shearer was leaving tailgate area towards the headgate a 4"" line was struck by shearer and the 4"" line pinched employee against a shield.
Employee was drilling into a metal bracket with 5/16"" drill bit when the bracket rolled over. The drill bit hit the left index finger. Four stitches were required to close the wound
Employee was operating shuttle car when he hit a hole in roadway and bounced up and hit his head on the canopy of the shuttle car.
Injured employee was assisting fellow employees carrying a 50' length of air line. He stopped and stated he had pain in his neck and radiating down his left shoulder. He was previously tightening 3"" threaded hose couplers. He finished the shift but has not returned to work.
EE was backing out of the #3 heading with a load of coal. He crossed under the roof bolter cable with the back of the scoop and then he turned his head to be certain the front would go under the cable then he ran over a lump of coal and it pushed him against top.
Laying on back knocking end pins on machine swinging 15 lb sledge hammer pain in abdominal area
A plant mechanic was standing on the walk way near the belt line assisting with the startup of the belt. As the mechanic turned to walk up the belt line walkway, he felt and heard a ""pop"" in the calf of his left leg. The mechanic also reported feeling extreme pain at the same time.
Working on radiator, burned on right arm with scalding water
Employee was helping move a continuous miner, when he went to move the miner cable out of the roadway a small portion of the rib rolled out striking him on his left foot resulting in a strain and a contusion.
Working on bus strained L knee climbing into drivers seat
Employee was installing a roof bolt in the rib on the 1 West section and as he was inserting the bolt into the rib he hit the top of his left hand on the drill guide. Employee felt pain and a knot raised up on his left hand. Employee worked fifteen days following the incident was off three days came back and worked seven days before being taken off work on Nov. 21,2013.
Bucket of forklift was full of rock when driver lifted bucket too high and rock dislodged and hit his left leg.
EMPLOYEE WAS OPERATING A SHUTTLE CAR WHEN BRAKES LOCKED UP CAUSING HIM TO HIT HIS RIGHT ELBOW ON THE PANIC BAR.
Individual was bolting on the right side of the miner when top of the roof/rib fell out striking him on the right leg and foot.
Employee stated that he slipped and fell to the ground while attempting to dismount a D10 dozer.
EE is a scoop operator, supplying the roof bolter. He went to get a water on the offside of the bolter. The rear rubber tire ran over the right foot-causing bruise. EE was wearing Metatarsal Boots.
Driving a haul truck; hit a pothole; bounced and hit his head on roof of cab
While removing bulldozer cutting edge, bolt sheered off and struck miner in the shin. This resulted in laceration and possible hairline fracture.
The employee was working inside of a hopper when a loader operator brought a load of material and dumped it on top of the employee and immediately his coworker began digging him out,uncovered within 15 minutes, he was taken to the hospital for testing and released ok. The next day he had a swollen ankle on his right foot and is being treated with modified duty release.
EE was doing EQ. maint. He was pulling out an anvil in the VSI when he felt something pull in his groin area. He went to the Dr. on 10/21. They referred him to a surgeon that appt. is 11/7. We will know more at that time as whether he needs rest or surgery, if he will be off work and how long he may be off.
Highway truck driver was exiting his truck, his foot slipped off the top step, fell into the truck parked next to him and then fell to the ground.
Employee was beginning to climb stairs with handrail from equipment area to quarry area. On first step he heard and felt his knee pop. Strained his right knee.
Three miners were traveling underground in a UTV. The driver pulled the UTV into a crosscut to allow traffic to pass when the UTV tipped onto the left side. The miner in back was bracing his left foot against the side of the UTV; when his foot slipped his leg was pinned under the left side of the UTV resulting in a fractured tibia.
Employee was placing metal in metal rack. While lifting metal he felt pain in his lower back.
Employee was sitting in off-road coal truck while being loaded. After loader operator dropped load from bucket, the operator caught bucket on truck bed, pulling off-side of truck off the ground causing driver to strike his knee on steering wheel of truck.
The man was roof bolting in a working place. He re-positioned the machine and was preparing to continue bolting. A piece of rock fell between the supports and struck him on the head.
The EE was in the process of completing his pre-operational inspection on a Cat D-11 dozer when he slipped off the dozer platform and fell to the ground. He was taken to the hospital and diagnosed with fractured ribs.
EE states he was working on rock duster and pulled the lid off to work on breaker and strained his back.
Was helping remove a steel stuck in rib when he felt pop in his right shoulder. He didn't report it to anybody til next day. Doctor did MRI and removed from work on 10/28/2013.
Driver was backed up to lime pile to dump. As the bed started up, one side stuck and caused the unit to go on its side. Driver got out through windshield. Was transported to hospital but was released. His side is sore.
Rock fell from roof striking him on the head and right foot.
During the removal of the stick jack on a PC300 Komatsu excavator, an oring seal failed releasing hydraulic oil pressure which struck the mechanic in the left forearm. This resulted in bruising and swelling of the injured area. After X-rays and a medical examination it was determined no fluid had entered the skin.
Injured moved ram jack off of shearer went to pick it up, to get the ram out of the walk way. He went to place it on top of shield toe he smashed his middle finger between ram and double eye when he was setting it down. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor.
Injured employee was changing out the receptacle on a set of scoop batteries on 1 North Mains. While loosening a bolt on the positive lead, the 6"" crescent wrench he was using contacted the negative lead causing an arc resulting in second degree burns to his right hand.
Employee was walking into 3D header and stepped hole twisting his right knee, on 10/15/2013. He went and had x-rays completed on 10/15/2013 and had negative findings. He then saw a orthopedic surgeon on 10/31/2013, which he placed him off of work until 11/04/2013.
Service technician was using our weight cart to move weights to calibrate their truck scale. The cart flipped and the service technician hit his head on the weights that were in the cart causing a gash on his forehead requiring nine stitches.
The employee was removing top conveyor rollers from a pallet when some of the remaining rollers fell and struck the employee on his lower right leg.

He was walking down the track entry at 10-50 wall. He struck his hard hat on the mine roof causing him to fall backward onto the ground. He began missing work immediately due to his injury.
A haul truck pulled to the side of the road and stopped to allow another haul truck to pass.The oncoming truck continued up the ramp and contacted the stopped truck jarring the truck and the operator.The technician in the stopped truck was diagnosed with an abdominal hernia as a result of straining and bracing himself.
Employee was lowering a shaft down into a cell and when the slack of the chain came out of the strap his right thumb got caught in the slack of the chain.
EE stepped out of pickup onto uneven ground, twisting ankle.
Employee stated he was walking down the main decline when he slipped and fell on his left shoulder.
The operator of the #3 roof bolter was attempting to put another cable bolt up (after having hung one) he started a hole adjacent to the previous hole when a piece of draw rock became dislodged from the top and struck him on his left wrist.
While driving haul truck, left side went into ditch and ee's left arm caught in steel bar on door, straining neck. Examined, diagnosed with cervical strain and put off work due to inability to operate equipment.
Employee reported that he was helping fellow employee true holes on the EL-Jay Screen when a bolt hit him in the mouth, chipping a tooth.
The employee was assisting in the bolting operation in the belt entry when the operator running the bolter set the stab jack down on the injured employee's right foot resulting in a fracture.
Employee was removing trash from the main plant with a forklift. Some of the trash fell off. He stopped to pick it up. He slipped off the step when he was getting off the Forklift. He caught himself before he hit the floor.
Employee pulled lower back muscle when he stooped over to replace his tracking reader battery.
Employee was removing a door on the head gate of the longwall in order to get to a sprocket. A rock fell from the roof hitting employee on the back of the head and shoulder area.
While deboarding a cable truck employee missed a step and fell onto his back. On 10/13/13 employee was evaluated in an ER and was taken off of work. He missed his scheduled shifts on 10/13 & 10/14. He returned to work full duty on 10/17/13.
Employee was bolting on # 2 UNIT WHEN A ROCK HIT HIS DRILL STEEL CAUSING THE STEEL TO HIT HIS RIGHT LEG.
The employee was bent over pounding stakes for a drill pattern. The employee felt pain in his lower back that radiated down his right leg. The employee was transported to the local clinic and the medical provider prescribed a prescription (muscle relaxer) which caused the employee to miss his next shift.
EE was operating D8N dozer on stripping dump area, pushing stripping material. While leveling dump area he encountered a slab of rock est. to be 6'x8'x15"". His plan was to remove it from topside of dump. He positioned dozer at or about 45 deg angle to the top of dump site. Dozer traveled over berm & traveled a distance down the slope then slid to left side, came to rest at bottom
Was guiding steel into chuck. Misalignment occurred causing a springing action of steel. Tore a laceration into left hand.
Employee was on his knees bolting and had to bend the bolt to install it. As he installed the bolt and pushed on the bolt to straighten it back out he felt his left knee pop. He went to the doctor that day, continued to work and began missing work on 10/22/13. (Strain to the left knee).
A piece of rock fell from where the rib meets the roof and struck the employee on the head and neck.
Employee started to ascend the ladder of the loader and allegedly fell backwards. Fisher Sand and & Gravel Company and out workers compensation insurance company are questioning the validity of this claim.
EE reported sore R knee on 10/12/13 attributed to using plant stairs on 10/11/13. Sore knee symptoms were treated conservatively and EE performed normal duties until arthroscopic surgery was performed for medial meniscus tear on 5/9/14--first lost-workday.
Snake bite to right thumb area
EE was putting straps together on conveyor chain. EE put a chisel between press and the conveyer chain. The chisel came out hitting EE in the left wrist.
A mechanic was installing a pin on a scraper hitch. The employee was using a sledge hammer to pound the pin into place behind him. When the hammer contacted the pin a piece of metal from the pin broke off and embedded into the mechanic's right leg. Employee was taken to ER but had to have surgery to remove the piece of metal from his leg.
Injured employee was pulling track 60# rails into a spur when the rail caught a tie causing the employee to experience pain in his lower back.
On October 10,2013 the Electrical Foreman was using Channel Locks to loosen a seal-tight connector on a 2-inch conduit when he felt pain in his left shoulder. On October 15, 2013, the employee went for a diagnostic exam. The doctor sent him for an MRI. On November 13, 2013, the employee saw a specialist who read the MRI and recommended therapy and a lifting restriction.
Employee was removing a portable pump - While removing hose, employee stepped on a rock and twisted ankle, causing his hip to jamb, injuring his lower back.
EE was getting out of the road grader and felt a sharp pain in his right knee. He ended up having to have knee surgery.
Employee was cleaning rock off of the unit power box in #4 entry on #1 unit when he felt a sharp pain in his right shoulder. A MRI done at a later date showed a tear in his shoulder.
Employee was lifting steel weights for a gravity belt take up to remove them. At the end of the shift he reported pain in the abdomen. He was referred to a doctor, FMC received the medical information on 10/22/2013 to show this hernia was work related.
Flipped Water Truck into water sump on hill grade.
Injured was helping a co-worker move a 4"" twenty foot piece of pipe and when his co-worker pushed on the pipe to walk, injured felt a popped in his left shoulder and pain. PPE was not a factor, mining equipment/system was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
Individual was stepping into 20 ton motor when he heard and felt a pop in the right knee.
EE was carrying bits to the miner and stepped around the boom of the miner twisting his back.
EE alleged that the bed of the Cat 789C hauler free fell and undestinational distance, causing an injury to his back. EE said he raised the bed of the hauler & then as he was lowering it the bed stopped, he pushed the down lever & the bed of the hauler; FREE FELL down against the frame. The hauler was checked by the mechanic & no problems were found.
EE was in process of installing roof bolt when bearing plate caught drill guide causing plate to jerk back catching employees left thumb and bending his left thumb backwards (hyper-extension).
While checking beltline with co-worker, co-worker went up to top of hopper to unlodge a rock that was wedged. Hard hat fell off of co-worker onto beltline below. Emp attempted to grab hard hat and ran, slipping, and falling on his Lt. side. Strain to Lt. Shoulder, continued complaints of discomfort, examination showed tear / Lost time on 01/29/14 when surgery was performed.
Injured was standing on the pontoon toes of long wall shield, pan was pushing his foot slipped off and got caught between relay bar and pontoon shields. PPE was not a factor, mining equipment/system was a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
Employee was helping install a new main gate post when he stepped off the side of the ditch beside the gate and twisted his right ankle.
Employee became ill during a molten zinc pour. The employee verified his PPE was correct and had proper air flow. Employee went to the emergency room and was released w/out restrictions. The employee went to clinic on 10/10/13 and was instructed to stay off work the following day if his flu like symptoms continued.
Employee was working on setting a pump unit #1607 in the southwest coal pit when he sliced the pinky finger on his left hand. He went to IMED on 10/9/2013. With his arm hurting. Worked part of this day.
Employee was walking outside the mine and stepped in some mud and pulled his foot out and twisted his knee.
Employee stated he was bolting on the #2 unit. He had finished the hole with starter steel and as he inserted the finisher steel he felt a pain in his lower back.
A rock became lodged in the auger tube. When the employee pried the rock loose, bolts securing the auger also loosened, causing the auger to fall from the sample tube. The employee suffered crushing injuries to his right arm as a result of this incident.
Employee states that he was helping to move a water pump. After moving pump he felt a burning pain in his knee. Employee informed company on 11/12/2013 that he has been going to physical therapy.
Employee was exiting the operators deck of a battery scoop. As he stepped out of the deck onto his left foot, his left knee popped and he experienced a lot of pain. He was unable to continue working due to not being able to put any weight on his leg. He began missing work the next shift.
OPERATOR PARKED HIS TRUCK AND PLACED TIRE CHALKS UNDER TIRES, STEPPED BACK TO GO BACK INTO HIS TRUCK AND STEPPED ONTO THE CHALK AND TWISTED HIS LEFT ANKLE.
Employee was collecting his tools for the night when he turned and his right knee gave out. He fell to the ground experiencing severe pain and unable to put any weight on the leg. He began missing work the next day.
Employee was running a 992G front end loader and was prying rock from the coal and the bucket slipped from under the rock and loader bucket came up and went forward into solid wall.
Haul truck operator was stepping out of the cab for a smoke break, slipped on some rocks on the deck, and felt a pop in his knee, and felt pain. He waited about an hour later and reported to his supervisor. He was taken the ER, taken off work until he can see an orthopedic specialist. CURRENTLY ONGOING.
EE slipped in mud while shoveling and felt a sharp pain in lower right back and hip. It was diagnosed as muscle strain, causing some lost time.
Stepped off of a ladder while shooting a center line and stepped on a rock rolling his right foot.

No lost time, worked 17 shifts before claiming he was hurting. On 11/4/2013, his wife called supervisor stating that EE fell at home over the weekend and would be off work a couple of days.
Employee was loading 54"" belt framing into a belt skid and felt a sharp pain in his back, he went to the doctor and had x-rays and a MRI. He is off work with a bulging disc.
Employee was hanging miner cable when he felt a sharp pain in his back and hip.
An Electrician was installing the #2 propel blower motor breaker/starter when the metal backing plate contacted the energized line side phase leads. This created an electrical arc flash that caused 1st and 2nd degree burns to the face and right hand of the technician.
There was a loose guard over a pulley, and the Miner didn't follow company and MSHA Safety Policies to shut down and LOTO the machine before attempting to repair. He attempted to make the repair while the conveyor was running, and got his fingers caught in the pulley, cutting two of his fingers badly.
Employee was moving quickly from an area when he tripped and fell over a cat head while going by the power center straining his knee and leg muscle.
EE was working on the installation of an overcast. While he was walking from one side of the x-cut to the other, he stepped on a small piece of rock causing his foot to give.
Ee parked haul truck in parking ditch, stood up and felt pain in low back. Seen by doctor, examined and diagnosed with low back strain, put off work. Subsequent follow up resulted in sedentary work release, but sedentary work not available so ee remains off work.
While attempting to load a sheet of steel onto a loader bucket the miner lost control of it and it fell striking him in the lower leg resulting in a broken right leg.
Injured was cleaning build up on belt structure with a wedge cap. The wedge got caught on top roller pulling right arm in between top roller and belt before he could pull belt pull cord to stop the belt. PPE was not a factor, mining equipment was not a factor, compliance rules/regs was a factor, miner proficiency was a factor.
Was working on putting a shuttle car conveyor chain back together. Co-worker was tightening come-a-long. The chain attached to the flight (not the come-a-long chain) broke loose striking employee in the hard hat.
Employee was getting ready to operate a battery powered mantrip. As he was setting down he slipped, causing his elbow to strike a can of WD-40.
Injured employee was changing a roller on the 1 North belt at 53 block. He stepped under a 6"" water line into the crosscut and stepped on an old roller covered in rock dust. This caused him to lose his balance and twist his right knee. Employee did not begin to miss work until 12/9/2013.
Two contractors were replacing screw jacks, in the process of handling one of the jacks, it slipped from one contractor's grasp and hit him on the foot. He has been placed on restricted duty.
Employee was stepping down off of a rock truck when he felt pain in right knee. Employee has been working since date of accident. EMPLOYEE DID NOT START LOSING TIME UNTIL 12/6/13 DUE TO HAVING SURGERY ON HIS KNEE.
EE reported 10/31/13 that a personal injury was a work related injury. EE believed while helping lift 4X8 wire screen on LS1 screen that he strained his back/neck. The EE cannot attribute any instance during this task which caused the injury. EE stated he was sore the next morning, however he did not attribute his soreness to his current injury which has worsened.
On 10-04-2013 An Employee was standing diagonal to the inclined walkway and was moving a super sucker hose to the side when he felt pain in his knee. He was taken into town to the doctor to be examined. He returned to work with no restrictions. On 10-29-2013 the knee was not getting better so the employee returned to the doctor, at this time he was given work restrictions.
A rubber tired personnel carrier rolled out of the adjoining break through and struck the employee's left leg between the scoop door and the frame of the machine resulting in a fracture above and below the knee. The steepness of the grade was a contributing factor.
The employee was drilling roof bolt hole his glove became caught between the two pieces of drill steel and twisted his hand and arm

A tails pump house employee was hosing down the floor when an old brace (pipe separator) fell from above striking the employee on the top of his hard hat. Employee received a concussion.
EE did not use his safety equipment as required by company policy. He had on his harness and tie off cables were attached to his harness, he did not tie off. He fell and broke arm and bruised hip.
Employee stated that he turned the #4 belt conveyor off by the remote switch and climbed onto the #4 belt to remove a rock from the #5 head drive and the # 4 belt started up and he was propelled into the belt crossover which is located outby the rock box of #5 head drive, which resulted in bruising to the left shoulder and abrasions to the back.
Employee was picking up a ventilation tube when he lost his balance. In trying to regain his balance he grabbed the rib protector that at the time was being lowered causing fractures to the ring and middle fingers of his right hand.
EE was pulling slack from shuttle car cable to re-hang when he was shocked. PPE: EE was wearing rubber gloves.
Employee was spotting timbers to be set. One of the timbers fell off of the trailer, the employee picked the timber up and twisted to toss the timber over to the rib when he felt a pain in his back. The injury resulted in a strained back. The employee is still off of work waiting on a follow up Dr. appointment.

Removing a boom pin from a shovel. Forklift was used to straighten the boom. A come along was used to keep the tension on the pin straight. When the boom straightened out, the pin came out and struck the mechanic on the foot, causing a broken tibia
The victim was attempting to pick a rock of the drag chain when the off-side rock picker attempted to bust the same rock with a sledge hammer. When the sledge hammer came in contact with the rock the victims left hand was still there and his pinky finger was smashed.
Injured employee stated that while standing on stacked blocks he twisted his back and felt pain in his back.

EE was switching a flop gate, the wrench he was using slipped off. EE fell and caught himself with his left hand resulting in a injured shoulder.
Employee was changing drawbars from 4 foot to 8 foot when the drawbar slipped and fell on to his right wrist causing a fracture.

Employee was inspecting a ""SK"" type valve on storage silos and requested another mechanic to assist in changing the valves position. Controls for the motor where on the opposite side of the pipes from the inspection location. Device was jogged 3 times and fingers were pinched in the valve. Assistant disconnected the mechanical arm to release, EMS was called and 1st aid was applied.
Employee slipped while climbing down a ladder and fell onto shoulder.
while traveling through a man door carrying a label maker, the employee slipped and fell, catching his leg between the door and the frame. The employee returned to full duty. an MRI was conducted and the employee required surgery and missed his first shift on 11/13/13.
Contractor employee working as a carpenter miss stepped and twisted his ankle. The injury resulted in a fractured ankle.

Employee slipped in a slurry spill as he was cleaning it up, and he twisted his right knee. He did not seek medical attention at that time. On 1/13/14 he requested to see the doctor because the knee was still hurting. On 1/14/14, employee saw doctor, x-ray was negative and he got corticosteroid injection for inflammation. MRI is being scheduled.

Employee was walking down some stairs and stumbled. After he stumbled he felt a pain in his hip. Employee did not want to go to the doctor at this time. The pain has not gone away and the employee wished to see a doctor on 11/14/2013. Employee has been put on restricted duty as of 11/15/2013.
(Weather - dry/calm) Accessed fall protection rack then activated air operated gangway to lower it to top of tank trailer. Operator then walked out across gangway to trailer to open lid. The gangway w/midrail started up on its own, exposing operator as he tried to grab it - lost balance and fell to ground.
Team member was exiting the excavator. When he stepped down onto the step he felt a pop and a sharp pain in his left knee. He did not seek medical treatment that day. He returned to 10/22/2013, due to pain, he went to the clinic to have knee examined. The doctor diagnosed torn right medial meniscus. He returned to work on restrictive duty on 10/22/2013.

The 9 wire lacerated his upper right eyelid. The employee had safety glasses at the time of the accident but was not wearing them.

Worker was holding a rock on cutter #46, when the stone was snapped his foot slipped and he felt a pain in his ribcage. His supervisor saw him asked him if he was hurt he said that he was, but he thought it would get better over the weekend so he didn't wanted to go to the doctor, but on Monday he told his supervisor that his ribs still hurt and he wanted to go and see the doctor.
EE and another ee where changing the oxygen bottle on a portable welder trailer. The other ee set his end of the tank onto the trailer. As ee lifted his end to stand the tank upright, the other end slipped of the trailer, jerking ee left shoulder.

Coming down off a ladder when he reached the bottom and turned he felt a pop in his back.
Employee was on the trailer, getting ready to set the first load. He stepped back and his foot went in a hole in the trailer. He lost his balance and fell backwards. His body was hanging off the side of the trailer and his foot was still caught in the hole. He has a severely sprained right ankle.

Employee was casting at the CR4 stockpile. When he curled the bucket back, it hit hard and caused the loader to shake, jarring his back.
Employee was pulling wires (working off of a ladder) and while pulling the wires the ladder shifted. A few minutes later the employee felt a strange sensation in the navel area. Check with doc and said to do nothing at time over past years he has had pain and now had hernia surgery.
Employee was stepping out of the Bobcat and he thought he twisted his ankle, when checked at the hospital he had a broken left foot. He was located at x-cut 24 of the Headgate #3.
The worker was loosening a nut at the slide gate on a hopper. The nut would not loosen at first, so the worker hit the wrench with is left hand to break the nut loose. When he hit the wrench a sharp pain went up his arm, which caused his lower left forearm tendon to become inflamed.
Employee was traveling in a jeep as a passenger on Cool Hill #2 track. At 54 x-cut an old trolley fell out of the roof and struck his leg. He fell backwards and strained his back.
Changing primary fuel filter on CAT 980F. Employee descended ladder and lost grip/fell. Employee landed on left hip/buttock. NurseLine called and advised self-care with ice/ibuprofen for pain/swelling. Case handled as first aid until employee requested follow-up care 10/14/2013. X-ray and CatScan were negative, but doctor put employee off work 7 days.
The injured cut his knuckle of pointer finger on left hand while checking screens on Fine Tower, middle screen.
Removing rock from chopper with hand (instead of hammer that should have been used and he was trained to use). Another rock came down and hit his hand as he was turning the rock to pick it up. Hit pinky finger/nail on left hand.
After performing some routine maintenance, the miner felt some pain in his lower left back. he strained a muscle which inflamed putting pressure on a nerve.
While driving haul truck employee hit a pot hole and jarred his neck.
The shift supervisor went to the iron feeder and noticed that it was clogged and raw material mixed with cement. Then the shift supervisor and the shift mechanic started to hit the material with a spear to break material formations and began to go down. They used a hammer and the spear together. After these work the supervisor began to feel discomfort in his left shoulder.
Employee twisted right ankle on the last ladder rung while dismounting equipment which caused falling to the ground and breaking the fall with right wrist
Pulling on hood to open for inspection. Felt burning pain in left forearm. D.O.T. Highway Truck. Driver went in for xrays. Has to have surgery on 10-18-13
He was setting up a welder getting ready to work on a bag house. He was working on laying out welding leads getting them up to the bag house from the welder. He went to walk down a small decline near the work area and slipped, fell on left side landing on elbow on concrete. The area was wet and covered in mud and product. It had been raining the night before and that morning.
Employee states her seat would bounce up and down while being loaded, then finally bottomed out, causing her pain in the neck. She was taken to the ER and released to her next shift. While on days off, 10/9/13, she went to her PCP, was taken off work, and prescribed therapy. Currently ONGOING.
The employee was drilling in advance of our mining operation. As he was pulling the core barrel out the puller broke causing the core barrel to fall. It struck him in the ankle resulting in a hairline fracture.
Employee was stepping off the crew bus at the dry at the end of shift and sprained his left ankle which resulted in a restricted duty release.
Lifting and carrying Omega blocks. Felt pain in lower right part of stomach and a knot came out.

Employee began pulling out a hose from a hose reel when he felt discomfort in his right shoulder. After a medical evaluation he was placed on restricted duty.
WHILE WALKING BY #28 SCOOP TRACTOR EE HIT HIS LEFT KNEE ON THE HOLDING KNOB FOR THE FLINGER DUSTER.
EE had energized a 4160VAC contactor and was walking away from it when an arc flash occurred. EE was transported to a local hospital where he was treated and admitted with 2nd and 3rd degree burns to his right arm and 2nd degree burn to his right ear. EE released from the hospital on 10/11/13 but remains off work.
Employee was installing a blind on the classifier sand shoot. The blind fits tight and required the use of a maul to install. When employee finished task he felt a sharp pain in his lower back. He was placed on restricted duty on 10/04/13
Dusty concrete slope, employee slipped and lost his footing, broke his right Fibula as a result.
Injured was standing on a platform 4.5 feet off the floor, attempting to attach a come-along when he lost his balance and fell to the floor.

Employee was repairing an axle trunnion and while setting it on the ground, left hand middle finger became trapped between the part and a bolt causing fracture and laceration to finger.
A Caterpillar 993K loader was being used to carry another 993K bucket to the maintenance shop. Once the loader reached the shop both buckets were left in the rigged position. A welder in front of the bucket proceeded to wash it while it was still in this position. The bottom bucket slid out injuring his foot.
EE was unloading material (grass seed) from the back of a pickup and strained her back while lifting/twisting. Injury reported 11/4/2013. EE was seen by a physician and placed on restricted duty for 6 days-and has since returned to full duty.
Employee was walking down the stairs when he reached the last step he twisted his right knee.

REACHED OVER ON THE BACK SIDE INTO A BLIND SPOT TO KEEP BOLT FROM SPINNING. HIT TRIGGER ON GUN. BOLT ROTATED PINCHING FINGER BETWEEN BOLT AND FINAL.
Employee was cleaning rocks from a plugged screen.
Operator was cleaning spray bar. Removed hose clamp while hose was still under pressure causing the hose to break free and strike operator in the chest.
Employee was hammering a bearing when a piece broke off and penetrated his left forearm.
Employee was pulling cable when he slipped in a mud hole and began to fall. He caught himself with his left hand and felt a burning sensation in his left shoulder. He has been placed on restricted duty as of 11/7/13 pending the results of an MRI.
Employee reported back pain while operating a haul truck.
The employees foot was pinched between the outrigger and wheel cover on a Atlas Copco Bolter when he stepped on it to clean the windshield. The employee received a sprain and contusion resulting in swelling to the ankle and upper foot.
On 10/31/13 employee reported a sharp pain in his left shoulder, but related the injury to a task performed on 10/29/13 when he states he developed a sore shoulder. Home care/FA instructions provided and normal work duties followed; however, on 11/13/13, employer discovered employee went to PCP on 11/11/13 and was given restrictions and Rx medication for shoulder injury.
Injured stated that when pulling a piece of rock from the top of his continuous miner, he felt a pain in his abdomen. He continued to work his regular job and refused to go to a doctor. On 05/13/2014, he was sent to a doctor who diagnosed a left inguinal hernia.
Employee was replacing the plow bolts on a cutting edge of a loader bucket when his hand slipped and was cut by the edge of the plow bolt.
Float Crew leader-man was installing bolts in flange of a 42"" HPDE pipe when the pipe shifted pinching his fingers between the pipe flange and bolt. Pipe was being held by a WA500 loader to align the flange ends. Employee was wearing leather gloves at the time. Employee received two small lacerations to his middle and index fingers.
While unloading belt structure off of the trailer, employee's glove got caught pinching his hand between two pieces of structure. No fracture but employee does have some restrictions through November 1st.
The employee was working on a #4 MMU outby entry mud hole. As he was pushing a scoop through the mud hole, with another scoop, the scoop bounced up and jarred the employees back. The employee was taken to the Dr and put on restricted duty.
Stepped forward to grab a bucket in the splitting room and banged his left knee on the corner of the table. The employee reported this to his supervisor on the date of occurrence but refused medical treatment. The employee said he did not need medical attention. On 12/13/13 the employee sought medical treatment on his own then notified the company that he sought medical attention.
Employee was installing permanent roof bolts when a rock measuring 14"" x 12"" x 6"" fell and struck the employee on the right wrist.
EE walked to a pallet to retrieve a small piece of angle iron and as he bent over to pick it up he lost his balance falling into the pallet. The employee put his hands out and the right hand went between the slats on the pallet catching his thumb and forcing it backwards. Surgical intervention required.
Employee was prying a plate from the cable trough on the longwall face conveyor, as he was prying the plate with a slate bar, the plate suddenly sprung free and the bar hit the employee on the side of his left foot.

Injured reported that while attempting to unplug a drain pipe, he walked between the water truck and the wash plant to access the drain pipe when he caught his right forearm on a piece of sharp metal protruding from a steel beam
Employee was changing conduit on the miner cable when he inadvertently cut himself. Injury required stitches.
Changing a tire on a battery scoop. He was guiding the tire onto the hub while a employee was lifting the tire in to place with a fork lift. The victim got his hand caught between the tire and the fender of the scoop, fracturing one finger and cut one finger.
EMPLOYEE WASHING OUT TANK CAR, LOST HIS BALANCE, SLIPPED, FELL AND HIT BACK OF NECK AND SHOULDER AREA WHERE D-RING WAS ATTACHED.
Employee states he was pulling on a suction hose of a water pump that was stuck in the mud and injured his lower back.
One employee was picking up the old crusher hammers from the ground and handing those hammers to EE. EE was then placing the old hammers on a pallet. The co-worker turned to hand another crusher hammer to EE when he noticed EE was on the ground in pain.

An employee was helping other workers clean material off of the 110 conveyor catwalk when he stepped on a rock, rolling his left ankle.
Employee was walking to get supplies and he slipped on the floor catching himself. His rear end hit the floor. He caught himself with his arm.
EE was walking up stairs of haul truck, both hands on handrails, lost footing, stumbled and hit right knee on grating. Seen by provider, released to restricted duty on next scheduled shift.

While removing belt sheave from pump, the sheave slid off motor shaft, hitting employee's left elbow.
EE was operating drill rig and wanted to check the level of heat coming out of vent system. The cab heater was not blowing warm air. He reached up to feel the air coming out of the heater duct and his fingers contacted the squirrel type fan. The fan cut three of his fingers on his left hand near the ends. His index finger was nicked and the other two required 12 stitches.
Miner was on his knees shoveling around conveyor for a period of time. He stood up and turned his body to leave the area. His left knee popped.
The EE woke up on the morning of 10/23/13 with sore left shoulder. He believes that occurred from installing a 7.5 h.p. motor the day before, this was a report only incident. On 10/25/13 he was still feeling soreness so we sent him in to a medical provider and he was diagnosed with a left pectoral muscle strain and prescribed physical therapy.
Large rock dropped into bed of haul truck and jarred drivers lower back. Driver told supervisor he was fine and left for home. He then complained of soreness at the next shift

Employee was striking a chisel with a hammer on a piece of stone when the stone opened and the hammer hit his left index finger, fracturing the metacarpal bone.

Employee was attempting to align a hanger plate which was in a bind when the bolt was being driven into the lower rib. The hanger plate sprung which caused the employee's left thumb to be pinched between 2 plates, resulting in a laceration and fracture to the tip of the left thumb.
EE sprained his ankle on uneven ground as he was exiting from a man-lift's operator's station. Physician has placed him on restricted duty with follow-up appointments scheduled.

Employee alleges he incurred an umbilical hernia while coiling up fan cable. This cable weighs approximately 12.6 oz per foot. While in the act of coiling up cable the employee was bent over at the waist, the employee stated that he felt cramping in his abdomen and stood up and felt a burning sensation and a slight bulge in his abdomen.

Driller departed from his safe lifting technique training and simultaneously lifted & twisted. This twisting motion resulted in the ""tweak"" of his back that was ultimately medically diagnosed as a back strain & muscle spasm.
Employee was welding a loader bucket and took his gloves off to use a sledge hammer and accidently grabbed the hot spot on the loader bucket. He received a second degree burn on his right hand palm.
While transporting employee's from Pete Bajo back to the dry at the end of day shift, the bus hit a bump and jarred the bus occupants around. One employee reported soreness in his back and requested to seek further medical attention.
while pulling the shuttle car slack in 20th East, Employee had secured a loop of shuttle car cable to the Mini Trac and walked away when he noticed that the nip was falling off of the machine. As he walked back to the machine, another employee signaled the operator to go, and the machine ran over employees right foot. EE was released with restricted duty thru 10/24/13.
10/17/13 - Employee finished installing rollers on conveyor. While walking back from task, stepped on rock and strained Rt. ankle. First-aid was administered but employee had difficulty walking. Transported EE to Dr. X-ray negative. EE given soft ankle support and restrictions. 4 days restricted. Reportable as of 10/17.
