There was a kink found in the hoist rope 6 feet from the slope car. The kink was removed and rope reentered into hoist car.

A roof fall occurred above the anchor point in the 4 West Return Air course at 18 crosscut. The fall does impede passage. The width of the fall is 16 feet wide and approximately 10 feet high. The length is unknown. The fall does not effect ventilation. No other 50.20 criteria applies.
A fall of the roof occurred on the #4 unit (5th SW Panel), inby the unit loading point, #4 entry-face, crosscut 39 (intersection. The fall measured approximately 20' length, 15' width, & 8'-12' height.
A roof fall occurred above the anchor point in the 4 West Return Air course at 24 crosscut. The fall does impede passage. The width of the fall is 16 feet wide and approximately 10 feet high. The length is unknown. The fall does not effect ventilation. No other 50.20 criteria applies.
The mill building passenger elevator stopped between the first and second floors with two employees inside. Employees were going to be stuck for more than thirty minutes; therefore, MSHA was called. The elevator contractor arrived on site, determined a control fuse had blown, replaced the fuse, and the employees were released. Total time in elevator approximately 1hr 17 min.
A roof fall was discovered on this date. The fall was located on the 1-West Main at crosscut #12 in the #1 entry. The fall measured 20' by 25' and was 10' in thickness. The area was previously supported with 5' Hy-tech bolts and 12' cable bolts. Affected miners were removed from inby the location and the primary escapeway was re-routed.
Retreat mining on the #1 Section in the #1 Entry. Lifts 1 and 2 had been mined and while in the process of moving to the 3rd lift, a roof fall occurred. The fall blocked the #1 MRS.
While cleaning rock from expansion joints in shaft a rock struck wire on the buffers knocking it loose disabling the elevator.
The fire boss on the owl shift discovered the fall at the start of the shift at approx..11:08 pm, called the mine foreman and the mine foreman called this in around 11:20 no equipment or personnel was involved in the fall. The fall is at 16 break on the 2 west belt.it is approx.. 18ft wide 40ft long and 7ft thick we are in the process of creating a clean up plan.
Unplanned roof fall in the B-Mains at 1 crosscut Inby spad #434. Fall occurred right above anchorage point, which was bolted with 6 FT. fully grouted resin bolts. At the time of the fall, due to being a weekend, the mine was idle.
Unplanned fall at 315 blk intersection 9 So. haulage. Approx. 28' by 21' by 11' high. Multiple slips in rubble, including small amounts of slicken-sided shale, and a large amount of white chalky/talc-like powder, that was very slick, allowing separation in rock strata above the anchorage. No problems with training, PPE, mining machines or compliance issues.
An unplanned roof fall has occurred at Cedar Grove Mains #4 entry 19-20 xcut that is approximately 6 1/2 feet high by 70 feet in length.
A roof fall occurred on the 10 Left section belt line on top of the section feeder. The fall was approximately 28'L x 16'W x 12'H. The area was bolted on 4'x 4' pattern with 8' Super-Twist primary bolts and two (2) 16' cable bolts spaced every 8'.
At approx. 4:00 pm an unintentional roof fall was discovered in the 3rd E Mains at XC 15 between the #4 and the #5 entries (1st South belt line). The fall was approx. 6-8' high, 20' in length and 19' wide.
A roof fall occurred in the #11 seam escapeway at crosscut #92. No injuries or ventilation short circuit occurred as a result of the roof fall.
Employee became stuck in the man elevator at the primary crusher. Employee was trapped in the elevator for approximately 1 hour and 23 minutes. Employee was released from the elevator at 3:03 a.m.
The Bulk Explosive Truck (Unit 5556) had just been washed & parked on ready line with the engine shut down when a fire broke out in the engine compartment. The Engine compartment and cab were completely destroyed. The truck was not loaded at the time. Fire spread to a second Bulk Explosive truck (Unit 5555) parked adjacent to it destroying it also .
While a contractor was digging a ditch along an access road they contacted a underground conduit, causing a power bump that knocked out power to the #2 hoist. Three miners were stuck in the shaft for 6 hours.
An unintentional roof fall occurred outby the 001 MMU in the #3 entry in-between spad #'s 18483 and 18428. The fall measured 50' in length, 18-20' wide and approximately 6' in height. There were no injury or equipment involved with the fall.
We had rope bolts, cribs in the area. Fault in roof.
The small two man elevator went down at 5:00pm due to the bottom gate losing connection with the gate switch.
At 10:30 pm on October 23rd, a roof fall occurred at the 1 North sump. The fall measured 60 feet long, entry width wide, and 8 to 10 feet high. The fall did not impede travel of miners or disrupt ventilation.
During non destructive inspection of the hoist rope it was found that the minimum diameter was below 6%. The hoist rope was changed.
Brake pumps are not actuating in auto mode. Hoist down @ 0755, contacted MSHA @ 0808 ( contact name : / report # 00064522 ) hoist reported up @1005 to MSHA ( contact name : / report # 00064548 )
The surface high wall having connecting underground mine portal entries, failed to remain competent in solid form, resulting in rock strata breaking away directly over top of the No.6 mine portal entry and damaging two rows of permanent roof support, having been installed in the underground mine roof(s) immediate portal opening.
approximately 2' of deteriorated black shale fell out in the area causing the bolts installed to fail, However the main roof was intact and sound. This area was mined in April, 2006.
Had power bump lost the power to the 1600 air doors one person could not open the man door due to high pressure agents the door the VR-1 Fan did not kick out therefore it took 2 people to open the man door. The main ramp was blocked so MSHA was notified
An unintentional roof fall has occurred on the P1-1 Panel at the #15xc in the entries of 9 and 10. The fall was approximately 100 feet long by 17.5 wide.
Electricians could not get the MG set to operate properly. It was identified that the brake control stick was not properly positioned. The brake controller was repositioned and the system was reset.
A coal smoker was discovered in an inactive pit area that would not be extinguished within 30 minutes.
IGNITION - Occurred on #2 Longwall (E-10 panel) while cutting bottom with tail gate drum at approx. shield 24. The flame was orange in color and went out immediately. Water was still applied to area with wash down hose for precaution.
A power outage was reported by the plant operator. Smoke was visible in NE Corner by east pond on property. It was determined the grass fire was caused by a downed electric line and area was secured; the fire dept and SW Rural Electric (SWRE)were called to the plant. SWRE secured the elec line and Fire Dept began to spray the flames. The fire was extinguished at 9:50 am.
An unplanned roof fall was found and called out by the weekly examiner while traveling an airway with an MSHA inspector. This fall was found to be 80' long, 8' high, and 20' wide. This was in the examiner's normal route of travel.
At 0840 Company became aware of fall of ground beyond support, reported to MSHA hotline & field office at 0847. A verbal 103K order was issued at 1447. 10/20/16 inspector arrived at site, starting investigation. 10/21/16 remediation plan was developed & approved by Rocky Mtn. District & field office. 103K was terminated, company will update MSHA on the progress/completion of plan.
ROOF FALL - Occurred on E-6 Panel belt line of the longwall recovery. The rock fall was above the roof bolt anchorage and was approx. 17 feet wide and 20 feet long.
The hoist at #1 slope went down at 11:15 am with electrical problems. The hoist motor grounded out causing a PLC failure. The problem was repaired and the hoist was running at 12:40 pm.
A roof fall occurred in the face of #9 entry inby spad +1740 on the #3 working section (Panel 30) No injuries or ventilation short circuit occurred as a result of the roof fall.
IGNITION - Occurred on #2 Longwall (E-10 panel) while cutting bottom with tail gate drum located at approx. shields 33 and 34. The flame was orange in color, lasted about 20 seconds and was extinguished with water from wash down hose.
Found an unintentional roof fall in the #4 belt entry at break #30 on the Back Mains.
While working in 1 South to correct a stone that had fallen in the walkway, the barometric pressure dropped drastically. The stone in the walkway was decreasing the amount of ventilation. The mine was evacuated due to a methane inundation in 1 South at 11:45 pm.
Bad roof conditions. We had installed several different supplemental supports. The roof fall partially covered the front ATRS of a Fletcher roof bolter.
An area in the 1 South Seal walkway was found to have a stone approximately 10'long, 4'wide and 4'thick. This was not above anchorage but the travelway was blocked temporarily.
Employee inside elevator car when it stopped between floors. Repair service contacted and restored functionality allowing employee to exit car about 50 minutes later. Inspection found an MOI contact that stuck. MSHA notified using hotline, then later updated.
An employee was entrapped in the preheat tower elevator between the 3rd and 4th floor for approximately 1 hour. The employee was in good health and maintained radio contact until the elevator contractor was able to lower the car to the next level and allow the employee to exit the car.
On 10-13-16 a roof fall occurred above the anchorage point at the three way intersection of #1 entry east mains and #1 entry 6 North. The fall is about 20' in length and 20' in width. The height could not be determined but was in excess of 6'. The cause of the fall was weathering and a plan was submitted to remove this area as an active airway. No other part 50 criteria applies.
While hoisting a roof bolter out of the mine the dolly which the bolter was loaded on came off track damaging several guide rollers. The slope was down from 6:35 am till 2:00 pm while repairs were made. The roof bolter and dolly went off track due to improper weight distribution.
Unplanned roof fall discovered reportable in 23W #2 entry between 2 and 3 crosscut. Fall was approx. 10' high, 20' long and 16' wide. Most likely due to presence of white chalky/clay substance in a geologic stratification layer 1-2' above anchorage which assisted in layer separation. No problems with mining machines, PPE, training, or compliance issues.
Hoist contact, Mine evacuated at 7:30 am, MSHA notified was notified at 7:30 am, evacuation complete at 7:46 am. Hoist was repaired at 9:15 am. Tested and miners returned at 9:20 am.
Retreat mining on the #1 Section. The CM broke down in the #2 Entry for approx. 2.5 Hrs. The pillar line began to work. Once the CM was repaired, the #2 jack was moved up approximately 5' and reset. As the pressure was relieved from the #1 jack, the gob area began to fall in the #1 Entry and rode over into the #2 Entry onto the jacks. The fall was approx. 19'W x 28'L x 6'H.
A FALL OF ROOF WAS DISCOVERED WHILE MAKING WEEKLY AIRWAYS EXAM. THE FALL WAS OBSERVED IN NUMBER 8 ENTRY IN A DANGERED OFF AREA. THE FALL IS APPROXIMATELY 20 FEET WIDE, 90 FEET LONG, AND 20 FEET WIDE. ROOF FALL WAS IN A DANGERED OFF AREA LOCATED AT SPAD 410.
The hoist oil pressure switch malfunctioned and caused oil to get hot and hoist car wouldn't travel up or down the slope.
#1 shaft hoist rope was rejected during non destructive testing removing the shaft from service.
Uncontrollable Inundation of roof and face material flushed into the panline and shield location around shield 55 on the longwall face. This unsolidified material has blocked passage of persons down the longwall face and in conjunction with bottomless floor material making forward progress of the longwall face improbable.
At approximately 9:15 AM a miner was cleaning up at face and bucket hit a miss hole which was set off. Hole was approximately 18 inches from the floor. No injuries were sustained. The 1.5 yard JCI loader had a portion of the bucket lip damaged.
#3 hoist kicked out of synch. #2 and #6 were available.
Had lost power went to see why, found roof fall located on 2 North East at Break 47 in the #5 Entry. Was reported, no injuries, did NOT impede passage ways.
#90 crosscut 3-2 and straight of #3 Roof Fall in Primary Escape way. The fall is approximately 70' long, 5' high, and 17' wide. All approaches to the fall area were immediately timbered, and the escape way was rerouted. The fall will not be cleaned up.
At approximately 5:10 p.m. the service hoist went down due to a malfunction in one of the relays causing the brakes not to release. All employees were evacuated to the surface in a timely manner. No person or persons were trapped or injured.
Employee was driving a fuel/lube truck and noticed smoke and stop to investigate and saw flames. EE exited the truck to safety & notified EE's supervisor of the fire. The truck was a total loss and fire could not be extinguished due to the danger of inflated tires exploding and the fuel that was present.
At approximately 5:10 a.m., the Spice Lick elevator was taken out of service due an issue with the elevator's E-stop.
Improper use of haul truck braking system. Total loss of haul truck.
Planned power outage. No one working in the mine during that time.
At approximately 11:45am two Millwrights were using the Blending Bins passenger elevator when it stopped moving. The Lehigh electrical dept. was called to the Blending Bins to troubleshoot the problem. The electrician reset an interlock switch in the drive room to restore operation of the elevator. The two millwrights were in the elevator approximately 45 minutes.
A roof fall was discovered at crosscut 9-20 on the mainline haulage. The fall is approximately 16' wide, 30' long and 10' high. Mainline haulage is blocked but ventilation is not affected. The area has been rebolted and is in the process of being cleaned.
At approximately 2308 on Wednesday, October 5, CAT793D HT953 right rear inside tire caught fire. HT953 was traveling loaded South on the North/South Haul Road just north of the Oasis water stand.
A pillar fall occurred on the active 002 Pillar Section that entrapped a Mobile Roof Support. Two MRS units were located in the No. 2 Entry and were blocked when rock fell from the left pillar lift and some of the material slid out in front of the MRS units. One of the MRS Units (# 3 MRS) was retrieved and the other (#4 MRS) was entrapped when the roof in the pillar line fell in.
A power bump at 5:04 am caused the site to lose power to our hoists for an extended period of time.
Crews 1&2 were riding elevator when car stopped at landing. Crew 1 exited car sending crew 2 to landing. Pushed down button that sent car into over travel locking doors. Crews were removed from car at 8:00am.
Roof fall 60 feet inby spad 50041 on outby Return of MMU-006 in the No 1 Southwest Submains along the No 2B conveyor belt. The fall was 8 feet thick and 150 feet long.
We had a roof fall on the active section of HG#5 at xc71 in the intersection of #2 entry. A cleanup plan was submitted to and approved by MSHA to clean up this area.
A roof fall occurred in the 00 West Aircourse. The fall is located in #1 Entry at the intersection of #6 Crosscut. The fall is approximately 50 feet in length x 16 feet in width and is approximately 9 feet above the existing mine roof (1 foot above the anchor point). The cause of the fall was determined to be weathering of the mine roof
Doing maintenance on coal auger welding & cutting on frame and heads, fire started. Location where it's too dangerous to get to fire extinguishers.
A roof fall occurred between 11-12 break on the #4 Main Line belt that spanned approximately 80' long x 18' wide x 8' high. The area was bolted with 6' super-twist primary bolts on a 4'x 4' pattern and two (2) 12' Post-Tension cable bolts spaced every 10ft.
Per MSHA Preliminary Report: The screen plant was being attached to a front-end loader in preparation of being moved into the pit. The victim was proceeding to hook them together when the equipment moved, pinning the victim. The victim was later discovered injured and leaning against the loader bucket. The victim died of their injuries the following day.
While removing outrigger pad from Boom Truck the employee lost their balance and their right pinky finger was smashed in between a piece of metal. This resulted in the loss of the tip of the employee's pinky finger.

Employee inhaled floor absorbent felt tightening of chest and difficulty breathing.
Employee stated EE was pushing a hopper under a scrubber when EE felt a ""pop"" and developed a bulge in EE's mid-section. EE was sent to a physician where EE was diagnosed with a hernia. EE has been sent to a surgeon to schedule the repair for the hernia.
Employee had set the CM up in #6 entry. EE was turning EE's body around to get better positioning, as EE turned EE tripped over a lump of coal and fell to the ground. this resulted in a fracture to EE's right elbow. EE has since had surgery to correct the fracture in EE's elbow.
IE was operating Roof Bolter in 5 Right location in face. In Process of installing second row of bolts. When piece of draw rock fell from the roof between ATRS and drill canopy. The rock struck employee's hard hat and cut employee between hard hat and safety glasses. Rock measured 21 inches wide x 19 inches long x 6 inches thick.
Fell backwards while trying to move an attachment for the skid steer by self.
Employee stated was pushing a hopper under a scrubber when employee felt a ""pop"" and developed a bulge in mid section. Employee was sent to a physician where employee was diagnosed with a hernia. Employee has been sent to a surgeon to schedule the repair for the hernia
Employee stated that EE was in the process of removing the cat pads from the 226CM when a grease plug failed. This cause part of the grease plug to strike EE in the face (left cheek). This resulted in lost work days and a laceration, which required sutures to close the wound.
Employee was pulling shield #5 from shield #4 on the I-5 longwall when a fitting on a hose broke hitting employee in the left knee.
Employee was preparing to hang a ventilation curtain extension when EE stepped in a hole twisting right knee.
Contractor cutting on arm of excavator, grease fitting & grease shot out hitting contractor. Contusion to right shoulder requiring 2 stitches, 1st degree burn to left side of face & 2nd degree burn to left collar bone both requiring first aid treatment. Contractor states was released from hospital with no restrictions related to this accident.
Miner tripped on grating, fell on hand.
Using Vacuum truck to clean up CKD dust around the Pallet Pan building. A large amount of excess dust moved and struck victim in face.
Employee was installing wall liners in PC1/PC2 transfer chute when the liner slipped from hand. The liner pinched left ring fingertip causing a laceration requiring 5 stitches.
SECURITY GUARD WAS WALKING OUT OF GUARD SHACK LOCATED AT BOTTOM OF HILL AND EE STEPPED THROUGH THE DOOR THRESHOLD TO THE PORCH AND TWISTED ANKLE.
A fully trained miner was tramming the roof bolter from 0 to 1 entry. The machine made contact with the rib line and a piece of metal on the canopy struck EE in the right ear causing a laceration. The injury was not result of any compliance issues nor did it involve any protective devices.
EE was climbing down water truck. Someone tossed gloves to EE. EE reached out to catch them, lost balance and fell.
Operator was walking in packaging area with the flow of forklift traffic. Forklift driver was carrying a pallet load of packaged product. Forklift driver struck operator on left side knocking operator to the ground. Operator suffered bruising, skin abrasions and pain in left shoulder.
Miners were assembling a mast to install on the roof bolter. The mast was laying on two saw horses. As they were installing the drive chain the mast rolled and the motor guard struck the miner on the shoulder.
EE received fractured ribs when EE failed to negotiate a curve in a haul truck underground.
Drill steel was stuck in drill pot so employee put steel against top trying to dislodge, and doing so bent steel. Employee then tried putting steel into hole that was drilled and was guiding steel into hole with right hand,Employee was using up-pressure only and during the process moved joystick causing it to rotate striking right hand.
Employee was injured when a sump that was being lowered into the basement with an overhead crane came into contact with a severely corroded pipe and broke the hanger causing the vertical pipe to fall and come into contact with the employee's ankle, fracturing it. The bolts on the hanger were completely rotten.
Miner was walking down a ramp and tripped on something causing them to fall down stairs of a cone screen. Miner said they were not sure what they tripped on and there was no debris on the walkway. Miner fractured right ankle after the fall. Miner fell approximately 4-6 steps from the landing. The miner was not carrying anything during the incident.
A slicken-sided piece of rock 78"" long by 20"" wide by 0-11"" thick fell from the mine roof in the #2 entry of the E-mains 001 working section. This rock struck employee on the left hand causing a compound fracture to the employee's fifth finger (little). The area was bolted according to the roof control plan with 6' fully grouted resin bolts and oversized bearing plates.
Employee removing drain plug from machine, leveraged self for additional force and bruised right chest rib. Diagnoses was Right Chest Contusion.
Employee was operating a dozer when they felt a pain that made it hard to breath. Taken to doctor and returned to work, after a few days they were still having the pain. Doctor took employee off to rest.
Employee received an electrical shock after touching a power line while cleaning out a drop chute. Employee was sent to the hospital for burns on both hands and an exit injury on the right leg.
EE proceeded to exit the water truck when EE missed the lower step with EE's right leg. This caused EE to lose EE's balance and fall to the ground. During the fall EE awkwardly landed on EE's right leg causing a sprain of the outside the lower leg muscle.
An employee was walking down the stairs and twisted their knee.
Employee was getting off of a buggy when EE's foot got caught on the buggy and EE fell into the coal rib and hurt EE's left shoulders.
Employee was building and plastering a brattice when some of the plaster got on EE's glasses and into EE's left eye causing scratches and irritation to EE's eye.
A fully trained miner was pushing a winch when a piece of metal slid off the winch straining the employees lower back. The incident was not the result of any compliance issues nor were any protective guards involved. The employee did not begin missing work until 02/09/2017 (15 Weeks Later).
Employee and witness were completing repairs using a hand-held hydraulic jack. Prior to its use, the jack was placed on a ladder. The employee bumped into the ladder, and the jack fell off the ladder striking the employee's foot. Employee suffered fracture of great toe. Employee was wearing protective shoes.
The employee was entering the cab of shuttle car and twisted right knee.
Miner operator was moving a 2/0 cable out of the roadway in the intersection of #2 entry. As operator was turning/ twisting heard a pop sound in left knee. The pain became intense after that point.
The employee was throwing miner cable to the rib when ee felt a pop in elbow. EE continues to work but is now receiving physical therapy for the pain in elbow. This is a NDL accident.
Stepping out of dump truck, started to fall, grabbed door handle/rail to stop fall, felt something in left shoulder ""pop"", let go of handle/rail and fell to the ground, resulting in severe pain in left shoulder.
Service truck driver stepped out of truck the wrong way and sprained ankle.
Employee stated was in the process of throwing a timber out of the scoop bucket, in that process EE lifted the end of the timber up and felt a sharp pain through back and groin area. Employee sought medical attention as was taken off work. This incident is being reported as a Lost Time Injury.
Employee moved a bulk truck, set park brake and preceded to exit the cab. Using three points of contact climbing from the cab as foot reached the ground employee stepped on a rock and started to fall under saddle tank and reached back with arms to catch themselves. Employee felt a pop in right elbow.
An employee was working in a manbasket of a powder truck loading a development round in the 4470 North Hanging Wall. The employees right forearm was resting on the rail when material fell from the face. The material fell onto the forearm causing a laceration and later identified a slight fracture.
Employee was walking to shuttle car when twisted knee in a rut. Employee worked till 10/28/2016 when employee was taken off work.
Rolled ankle on rock on the ground while walking through pit.
Injured employee was helping another employee get a screen loose on the pep screen. They pulled on the screen and when it came loose, injured employee's right hand was mashed between the lever and the pep screen. Injured employee remains off work pending further doctor evaluation.
Employee felt a pop in lower back when employee and another miner were lifting a bottom roller up into position during a belt move.
Employee struck side of head on conveyor frame as EE walked beneath it.
Employee was welding on a loader bucket with a partner and experienced arc flash. Was using a Miller Welder.
Employee was walking up to the CM when EE tripped over the Loading Machine cable and sprained EE's left knee.
Employee stated EE was lifting the continuous miner power cable above head to hang it against the mine roof when EE felt a sharp pain in the abdomen. This resulted in a hernia. The employee began missing work due to this incident on 12/2/2016.
In the packaging area one operator got faint due to previous medical conditions, when operator fell injured one finger of right hand, needed stitches in finger. Operator has previous medical conditions which are under medical investigation for the faint.
Employee was checking the fire suppression on the miner, when the water inlet hose came loose hitting employee in the eye causing injury to right eye.
Replacing a motor on K660 coal drag. While lowering the motor to the ground it got caught on a mounting bracket causing the motor to roll, resulting in side-loading the gate on the hook of the Tuggit. Failure of the gate caused the motor to fall. In the fall the motor struck the removed guard causing it to slide into the employee's left shin and right ankle.
The employee was operating a haul truck, hit a hole in the road and jarred neck.
While moving the roof bolter, a piece of roof rock measuring 13"" x 15"" by 2"" in thickness fell striking employee's hard hat causing the hat to slide down employee's face. Employee sought medical attention on a later date and began missing work on 2/9/2016 when employee had surgery on their neck.
The employee was entering a rail car to clean out material. While stepping down from bottom rung of ladder the employee rolled and injured ankle. Treated as first aid initially, but employee indicated days later that healing was not progressing. Sought medical care on 10/31/16 when learned there is a fractured bone in left foot.
Employee was tramming a scoop when EE hit a bump in the roadway causing EE to hit EE's head and jam EE's neck on a scoop canopy. The employee complained of headaches and was diagnosed with a concussion and taken off work on 10/28/2016.
Trackhoe op. called supervisor to exchange trackhoe. Supv walked new trackhoe to the cut. Supv parked new trackhoe beside trackhoe that was in production. Supv was attempting to walk trackhoe off cut & moved tracks in wrong position. Machine walked off 8' cut & overturned trackhoe as a result. Supv slammed rt shoulder in cab w/seatbelt on, breaking shoulder socket.
Employee felt pain in right knee while shoveling coal.
The employee was lifting a steel beam with other workers when EE felt a burning sensation in groin area. The following were not a factor: Rules, Regs, Skills, Proficiency, Training, Equipment, Protective Devices and Clothing. Attitude may have been a factor. The employee began losing time on 10/25/16.
Employee was changing a hose on the take-up power pack located at the #5 Belt Drive when the wrench EE was using slipped. Employee was supporting self with left arm. When the wrench slipped body moved forward dislocating left shoulder.
While operating shuttle car, employee was bounced around in the operator's deck causing neck to pop. Employee experienced pain in lower back and left arm.
While roof bolting the employee was hit in the head, neck and shoulder by a piece of roof slate.
Employee moved shuttle car tire out of way with help of another employee. Bent over to pick up a pogo stick and felt pain in back.
While building a bridge, employee felt muscles tighten up in right lower back. This did not involve any compliance issues. No equipment involved. Personal protective equipment was being used. This did not involve job proficiency, training, or attitude. Employee did not miss any work until 10/31/2016.
A fully trained miner injured left knee when walking in the track entry of the 15 East section. No mining equipment was involved. The injury was not a result of any compliance issues nor did it involve protective devices.
MWT was working in maintenance shop. While seated on a stool ee reached forward to retrieve a wrench with left hand, and ee felt a pull in low back. EE was transported by ambulance to St. Elizabeth's ER where ee was treated and released to home later that evening.
Employee was shoveling on plant feed belt & picked up big load of mud on shovel, when turned to put on belt strained back. Cause of injury to big of load & body positioning. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/28/2016.
Employee was using a sledge hammer breaking apart a miner chain when a piece of metal sheared off & hit employee on right pointer finger knuckle.
The employee was bolting and while putting the roof bolt into the hole a piece of rock hit the employee in the right shoulder. The employee was injured on 10/21/2016 and continued to work until the employee went out for surgery on 11/16/2017.
Customer/end dump truck driver climbed up on loaded trailer to fix tarp. Driver fell from trailer breaking right hip.
Removing belt structure at 2E longwall tail, using a hammer to beat out a pin in the structure, with EE's head positioned under the structure. A piece of debris (metal/rust/dirt) fell, bounced under EE's glasses and landed in right eye, resulting in a corneal abrasion. No problems with mining methods, training, PPE, or compliance issues.
Employee said in the first 15 min of start of shift needed to move the gun and pulled something in lower back.
Employee was pumping ""glue"" into the mine roof near the 11 Rt longwall face in the belt entry. As EE pumped the glue into the roof, a slab of rock/headcoal about 3'x4'x1' thick fell out between existing roof straps. The falling rock struck EE across back. EE stated that EE felt a pulling sensation in back. EE was brought out and transported to the hospital.
Employee had unloaded supplies off fork truck. Went back & stepped onto fork truck, got to second step on ladder, turned & foot slipped, toe went into mud on ground & fell forward. Cause of injury wet, muddy conditions. Incident didn't involve any aspect of compliance with regulations/rules/PPE. Miner proficiency may have been a factor. Started missing work 10/24/2016.
An employee was operating a underground scoop loader to perform clean-up work in the P-6 tunnel. When EE was approximately 500 yards inside the tunnel from the tail end side, the right rear tire blew. The employee stated that something, EE was not sure what, knocked off EE's hard hat and respirator.
Employee was carrying a timber to be set on the belt line due to wide bolts. As employee lifted and began to drag the timber employee felt a pain in lower stomach/groin area. The employee did not receive medical treatment until 12/2/2016, in which employee was diagnosed with an Inguinal Hernia.
Employee was drafting barge and when boat came to face up barge there was slack in tow line when boat faced barge it caused the tow line to whip striking miner in back and fell into loaded barge. No other part 50 criteria applies.
Employee felt a pain in left shoulder after bending a roof bolt while trying to install it into the mine roof.
Employee had just dropped underground and was getting on 6 ton to go to section when they twisted their left knee.
Employee reported that while looking for hydraulic hose leak on Gill Beetle Drill, EE's finger was grazed by hydraulic oil stream.
Employee was tightening a bolt on Cat D-9 Dozer track return idler that EE noticed had fallen out. While pulling on a wrench, felt a sharp pain in right shoulder. EE continued to tighten the bolt and then went for some oil to put in dozer and when EE picked up a (5)five gallon bucket of oil that had about (4)four gallons in it EE states pain in shoulder was worst.
While changing rollers on #2 Main North belt, employee felt pain in stomach and noticed belly button was pushing out. This did not involve any compliance issues and there was no equipment involved. Personal protective equipment was being utilized and this did not involve job proficiency, or attitude.
Employee was changing bucket teeth on a PC 300 excavator bucket on October 19th 2016. The employee states EE felt a pain in EE's right shoulder while swinging sledge hammer and needed to stop for a minute. EE requested medical treatment on Nov. 4th. EE was diagnosed with sprained right shoulder.
Employee fell approximately six feet from an extension ladder while putting supports under a belt line at the crusher and injured right foot/heel.

Worker walked into a safety screen while checking a problem with the portable plant. Worker was wearing a hard hat and safety glasses and still scraped upper cheekbone, nose and eye.
Injured was examining fire suppression bottles on the #9 locomotive when the injured went to the lower the lid, the lid fell and struck the right thumb, resulting in a laceration requiring stitches.
Hot metal came in contact with lower leg after cutting with a torch.
The CM was not being operated consistently with the training EE received. The EE was not located a sufficient distance from the CM and received injuries when the CM contacted the EE. Witness interviews and detection system logs also indicated that the EE was not wearing the proximity detection locator. Appears the EE placed locator on CM instead of wearing it on self.
Employee was walking along the longwall stageloader when they stepped on a hydraulic hose valve. Their foot apparently activated the valve & the loose end of the hose abruptly snapped back & struck them in the face. Employee finished their shift. The employee visited a hospital emergency room the next day. Employee stated that the medication they were placed on made them ill.
Employee was using an air chisel to descale a box in Precipitation. While doing so, the air chisel shifted and dropped, catching the employee's finger between the chisel and metal structure resulting in a fracture and laceration.
Employee stated that after dumping load of coal on the feeder ee started back toward the miner. About 15 feet from the intersection inby the feeder the shuttle car breaker knocked causing the park brake to engage. The employee was thrown forward and in an attempt to brace self stiffened causing pain in lower back and down legs.
Employee was accessing the discharge end of the L3 furnace when EE's foot got stuck on a rail causing EE to lose EE's balance, resulting in a right ankle sprain.
Employee was using a wash-down hose to clean the area around the continuous miner ""CM"" stab jack, which is located beneath the CM tail. As EE was positioning self to better utilize the hose, they struck right ear on a piece of loose rock that was resting on the tail of the CM. Employee began missing time on 10/19/2016.
Employee was adding transmission fluid to a dozer. As employee was pulling hose for slack it became entangled causing employee to lose balance and fall from the dozer.
Using Fletcher center bolter in St Leo rehab area to drill hole in roof. Installing second steel with right hand, holding first steel in roof, and dropped first steel onto right thumb, causing a laceration. No problems with mining machines, training, PPE, or compliance issues.
As the employee finished loading trucks, EE backed over some material EE had spilled earlier. EE felt a pop and sharp pain in EE's lower back.
The injured employee and another employee were picking up a bent dozer blade. In the process of removing it off of the face, they leaned it against the handrail, pinching the employees right middle finger. EE received 12 sutures.
They were moving a feeder when it hit the rib. They stopped the feeder and had to move the cable and when they move the feeder again the rib rolled out and hit an employee in the left leg.
Employee leaned on handrail of walkway and it broke, causing EE to fall several feet to the ground and break EE's arm. Accident originally reported to OSHA under SER 56522 before it was know this was a MSHA-regulated location. Reporting now due to oversight of reporting to MSHA at the time.
Craftsman struck in lower left leg from wheel chock that was run over and projected into craftsman, in opposite bay at fuel island fracturing leg bewteen knee and ankle
Employee missed a step on a ladder and then noticed knee popped a few times. Employee initially refused treatment, but at end of day noticed swelling on knee. Employee went to the ER, referred to orthopedic specialist and taken off work. Specialist released employee to full duty 10/26/16.
While loading rock duster employee felt pain in lower back
EE was out side moving a rail car with hand movements and ee slipped and fell, and hurt neck and back.
Driver (Sun Coast employee) fell from 2' height when handrail on a tank platform/walkway driver leaned on broke. Driver fell to the ground and broke upper arm. The handrail was constructed of wood and was in a weathered condition.
An electrician was injured while trouble shooting a controller bucket rotary disconnect for #1 dust collector fan. An arc flash resulted causing burns to employee. This is reportable because it is a lost time accident.
Injured was washing at the bottom of the dump/slope belt. Material being washed consisted of pumpable crib material that had been mined on the previous shift. The hardening agent of the cribs caused chemical burns to the lower legs.
The injured was on 4 East - No. 3 entry when the accident occurred. The employee was attempting to go through a stopping door. Got the door open about 5"" & it shut back, when the door shut back the center of the stopping came out. The injured was knocked to the mine floor & some of the blown out block landed on EE. The injured suffered bruises to lower right leg & left ankle.
Employee stated was operating a scoop along the #2 Roadway and hit a hole, jarring employee inside the operators compartment. This resulted in a back strain and lost work days.
Employee was roofbolting when finger got caught between the canopy and the TRS resulting in a laceration to the tip finger requiring sutures.
Employee was exiting a 980H loader when they slipped and caught themselves with right arm, hurting right shoulder.
The employee was a passenger in a jeep which collided with a 20 ton motor on the track haulage. The 20 ton motor was stopped and the driver of the jeep did not see the 20 ton motor in time and when driver hit the brakes the jeep slid into the 20 ton motor.

While conducting an inspection/cleaning of the loader bucket, the operator had not completely lowered the bucket to the ground. The hydraulics slowly lowered while parked, causing the bucket to pinch the operators toes on left foot between the bucket and the ground.
Employee was getting into scoop when left leg slid back and knee popped. Felt pain in it.
Temporary mine employee was standing on second ladder rung and ladder twisted causing injury to employee's right ankle. Lost time.
Employee was about to change drive belts on the screen and lost footing. Put left arm out and dislocated shoulder.
A hillside collapse due to a geological anomaly. The equipment operator had made approximately 10 passes at the new cut. As the equipment operator was backing up to make another pass/cut, the hillside collapsed. The force pushed in the window and partially filled the cab of the dozer.
Employee pulled a shovel out of the storage bin and felt a pop in the middle of EE's back. EE was lifting the shovel straight up and out of the bin. EE did not twist or bend when EE felt the pop. The doctor diagnosed EE with acute thoracic strain.
Employee stated EE and another employee were using a slate bar to pry a rock off the skirt board of the tailpiece. When the rock came free, the slate bar slipped, and struck the employee on the inner part of right knee. This resulted in a contusion. The incident became reportable when the employee began missing work on 10/21/16.

An employee was running the remote continuous miner when a piece of roof slate fell and struck the employee in the neck and shoulders.
An employee at the Indian Creek plant was injured on Thursday October 13, 2016 when ee lifted a five gallon bucket containing approximately 15 lbs. of window washing solution. Employee received treatment from a chiropractor on Friday October 14, 2016.
Operator was performing task of changing out condensate filters & was in the process of putting the condensate pot back into service. When operator started opening the condensate valve to fill the pot, hot condensate started spraying out from the pot gasket & made contact with right abdomen, right hip, both thighs & buttocks. Initial diagnosis was 2nd degree burns to all areas.
The miner became agitated and punched locker. Hand began to swell and miner left work and reported to the local medical treatment facility for treatment. Miner was treated for broken bones in hand. This incident was deemed to be ""Horseplay"" and non-work related after consultation with a workman's compensation representative.
While using a chain block to line up a trunion ball on a grader, the hook came undone and struck the employee in the groin area causing swelling.
Employee was using a pry bar to change a roller. When employee pushed up on the bar they felt pain in their back.
While employee was standing on the off-side of roof bolter #13, employee was struck in the right side of head and face by a rock that fell from the top which was 9' high. Injured went up to the bolter to ask the bolt crew if they needed any supplies when the rock fell from the top. The rock measured 42""long x 18""wide x 3"".
Employee was shoveling coal at Nole belt tail piece. When EE felt pain in left shoulder.
Employee was lifting a Curtain Liner into place in the H.S.I when it slipped from hands. EE was able to catch it before it hit EE or dropped completely. Two days later on 10/13/2016 EE started to suffer from bad stomach pain then later after the shift EE noticed naval was bulging out of Stomach. The Employee is Temp Employee we hired through the local Temp service.
Clear day. Employee was attempting to climb off loader to stop feeder for VSI Crusher. Employee slipped on step at rear of loader and fell injuring right knee.
10/11/2016 employee dismounted the dozer and turned to take a step and felt left knee pop. Employee kept working, but knee continued to bother employee off and on. Employee opted for surgery and became a lost time 1/2/2017.
On 11/23/2016 EE informed the supervisor that EE strained shoulder while performing ropes training on 10/11/2016. EE could not identify a specific event that cause the strain.
Rope broke on raw coal magnet while employee was pulling it over to clean it off causing EE to fall and twist ankle.
Miner was standing outside of the heavy equipment shop going through task training on a front end loader and was drinking a soda. A bee stung the employee on the lower lip. The miner had an adverse reaction to the bee sting and was transported to a local clinic where employee was treated for an allergic reaction to the sting.
The miner was operating a crushing spread. The miner went to the top deck of the cone crusher to check if the cone was operating normally. The miner didn't follow proper procedure in this case. The miner looked into the cone when there was only a few rocks in it and when EE did, a rock bounced out and hit EE in the nose. The miner was seen at urgent care for a nose laceration.
Employee was walking out of scale house and fell through grate scraping right leg resulting in a laceration.
Employee was picking up brattice block and heard a pop in right bicep.
Employee was walking carrying a one hole board when ee experienced pain in right knee. The area was free of extraneous materials and the mine floor was in good condition. This did not involve any compliance issues. Personal protective equipment was being utilized. This did not involve job proficiency, or attitude. There was no equipment involved.
On 10/10/2016 employee stated was working on the tracking and communication system in the # 6 entry, while walking out of the # 6 entry employee stated a rock measuring 1' 6"" x 1' 6"" x 3"" thick fell on right shoulder. Employee sought medical attention and was taken off work. This incident is being reported as a lost time injury.
After a rain event, haul truck left the stockpile, began to slide, doing a 180, and came to a stop. 2nd truck came off the dump, attempted to stop, slid sideways into 1st truck. 2nd driver was seen at ER on 10/12 and fully released. Oct. 15 employee complained of back spasms, went to PCP, and taken off work. CURRENTLY ONGOING.
Employee was stacking block for brattice when coal rolled off rib striking employee on right hand and thunb.
Employee tripped over a wash down water hose, stumbled and fell.
The Employee stated: that EE was bending a roof bolt on a flat surface, the roof bolt twisted around while EE was bending it and EE felt a strain on the right side of EE's lower back.
Employee was putting EE's 1st roof bolt up on the outside in #5 entry when a piece of rock fell between 1st bolt and the rib striking EE on EE's left leg.
Amendment for narrative: Employee was moving two empty railcar's using the rail car puller when something on the rail car puller cable assembly caught on a metal plate. This caused the metal plate to dislodge from nesting place striking employee in legs.
While the employees where traveling out of the mine the floor mat wadded up in such a way that it caused the throttle to stick and maintain speed when braking was necessary. The vehicle collided with the rib and resulted in a shoulder injury.
Company employee (not mine employee) slipped while exiting excavator and cut right palm on a piece of metal bracing concrete barriers for pond ee was removing silt from.
Employee was helping install a sand wall, when EE moved the bottom cap to square the wall up EE felt a pop in EE's left forearm. The employee was taken to the local medical facility for evaluation.
Employee was on overtime. EE stepped in a hole in the haul road injuring knee. EE did not seek treatment until 10/10. EE followed up with a Occupational Health Facility on 10/12 (next available appt.) & was granted a full release. EE worked the next two scheduled shifts. EE returned to the ER for re-evaluation on 10/14 & was place off work pending an MRI.
The injured was removing belt structure from the longwall belt on 26 right - No. 4 entry - break No. 6. The structure rolled and EE tried to catch it when EE's left knee buckled and popped.
On 10/10/16, Supervisor noticed that EE's right eye was red. EE stated that EE did not know what had caused the redness in EE's eye. On 10/12/16, EE went to see an Ophthalmologist who removed a piece of metal from EE's right eye. On 10/14/16, EE told Supervisor that EE was using a hammer to loosen material in a hopper when EE felt something in EE's right eye.
When installing a desktop, screwdriver slips causing a cut on left hand. This accident required two stitches.
Employee was assisting crane operator pull samples with the single strong back. While trying to align the single pull in the designated cell, the employee states that they put both hands on the block of the crane and received a shock.
Maintenance helper holding a plate on a bearing being installed on a screen was struck in the left thumb with hammer by other worker resulting in 6 stitches
A fully trained miner was injured when moving 15' beams. While moving the beams the individual felt a pain in their upper back. No mining equipment was involved. The injury was not the result of any compliance issues nor did it involve protective devices.
Employee states they were picking up and moving a case of water when the scales went off. Employee went to put the case of water down when they felt a pain in right shoulder. This was not reportable until 10/20/2016.
Employee was moving scoop tire with a slate bar, the bar slipped causing the employee to fall against the concrete.
Employee was unloading a bundle of belt stands when the belt stands fell off of the flat car and struck the employee in the right ankle.
Employee stepped out of operator compartment of supply motor and twisted left ankle on rib sloughage.
Contractor was shoveling in the slope, lost footing and fell on right side that resulted in a bruised lung. This incident is questionable and still under investigation.
Employee was lifting a box of parts when employee felt pain in lower back.
A fully trained miner was injured when carrying and unloading a high voltage cable. No mining equipment was involved, the individual felt pain in their back. The injury was not a result of any issues with compliance or protective devices.
The loader operator was traveling across the main yard and struck a pot hole. The impact jarred lower back.
Injured was on opposite operator side of the roof bolter. EE was bending roof bolts for the next row. The roof bolter trammed forward and trammed across a rock causing the rock to move. Injured was standing by the rock and the movement caused Left knee and ankle to twist. Suffered a strain/ sprain.
IE stepped on a rock causing EE's left knee to buckle and pop. EE continued to work up until January 18, 2017 when EE had surgery to repair an acute lateral meniscus tear of the left knee.
While walking thru the last open crosscut to the longwall, the employee stumbled, tripped and fell resulting in a simple fracture to the employee's right ankle. The employee did not violate any rules, regulations or policies. The employee had on the appropriate PPE. The employee was adequately trained and exhibits a good safety attitude.
EE told supervisor that EE had taken hard hat off because EE was hot and sweaty. After taking hat off, EE went back to work. EE's head was bent down, when raising up, EE hit head on the classifier tank-part of the wash plant.
Driver had gotten out of truck WAS STANDING IN FRONT OF left front axle, when another truck approached from the rear contacting trailer and pushed parked truck forward pushing truck onto employee's foot and axle. Damage to parked unit was broken tail light. Other unit had damage to tarp arm.
While moving a drum of air compressor oil using a hand truck, employee felt a twinge in the left side groin area. Employee kept on working. Later in the morning pain developed and a bump appeared in the same area. Note: injury IS NOW REPORTABLE DUE TO SCHEDULED SURGERY ON 11/16/16.
Employee reported being struck by some roof coal. Six weeks later EE began complaining of neck pain. An MRI was done and revealed no acute injuries but did show chronic conditions due to three fused discs, metal plates and screws in the neck. EE was taken off work on 1/12/2017. This is being reported as an Occupational Illness.
Employee tripped over a rope and fell in the water. EE did not report an injury until 12/14/2016. Workers Comp. carrier denied injury on 1/12/2017. After investigation it was approved on 1/27/2017. Vulcan was notified by letter on 5/23/2017.
Employee was installing supplemental roof bolts with a hand held roof drill. The drill steel caught, causing the drill to twist in grip. The twisting action caused a strain/sprain injury to right hand and arm. The employee worked shift on 10/05/16 but started missing on 10/06/16.
Two middle fingers were caught between derailer and railroad track when switching derailer to closed position. Employee sustained a cut to third finger which required two sutures and the fourth finger sustained a hairline fracture. In accordance to company policy, the employee underwent drug and alcohol testing which yielded a failed result.
EMPLOYEE GOING DOWN SLIGHT INCLINE, SLIPPED AND BUCKLED RIGHT KNEE TRYING TO CATCH HIMSELF.
Employee had driven to look at the sump and parked on a slight grade. Employee stepped on uneven ground getting out of truck and extended the right knee and twisted an ankle. On 12/19/2016, employee had surgery on the knee.
After performing routine maintenance on the scalping screen, employee was exiting via a 9' fixed vertical ladder when lost footing and fell from the 3rd rung of the ladder to the platform below. Employee was transported to the emergency room and is currently losing time.
Employee was pulling on a hose and allegedly sat on or strained testicle
Employee was changing a tire on a shuttle car. EE was working the tire off the wheel unit, the tire fell on EE's right foot.
Report only: CAH reported to Gulf Coast for return to work after being off for personal reasons. CAH informed Gulf Coast staff that EE's injury was related to work activity. No specific event was identified that led to the injury. CAH remained under the care of EE's personal physician until EE returned to work on 10/12/2016.
While loading a ripper tip on truck, mechanic placed hand in the line of fire, and the ripper tip cut and fractured ring finger. CURRENTLY ONGOING
While travelling through the curtain, the scoop and shuttle car collided.
Employee stepped off of a skid steer, slipped and fell in the mud.
Water truck driver reported a noise while steering to the right. Pit mechanic installed new grease lines, the mechanic instructed the water truck driver to steer the wheels from side to side as grease was added. As the wheels were turned to the right the mechanic was pinned between the tire and frame causing injuries to the mechanic, 4 broken ribs and a broken collar bone.
Ee was pulling on a hose and hit elbow on an I beam. EE was seen at the clinic and fluid was drained from the elbow and a steroid injection given. Ee put off work until 10/10/16.
Employee was performing a routine plant inspection. Employee was stepping over the railing into the screen deck area. Employee twisted knee while performing this task.
Employee reached down to pick up a hose and said they tweaked their back. Employee was going to place the hose nozzle in a position to wash sand from beneath a tail pulley.
Employee tripped over a cart in a well-lit walkway while trying to find a local disconnect switch. The employee fell over the cart and unto right shoulder.
Employee was using a wrench to loosen bolts on a valve on top of the cement silos. As employee was pushing down to loosen the bolt felt a pain in right shoulder. There was no damage to the shoulder but was issued muscle relaxers.
Employee works indoors in the Chilton Manufacturing Center as a Splitter Operator. The employee was operating a splitter and a piece of rock fell off the table and struck foot. The employee was wearing steel toed boots with metatarsal protections.
White side supervisor was in the process of switching fill pumps when the surge tank overflowed. The sump pump could not keep up with the flow. The liquor level rose quickly and entered EE's boots resulting in thermal and caustic burns to both lower extremities. Transported by RN to Baton Rouge General ER and hospitalized on Burn Unit.
Employee reported jammed finger on 10/30/16 and refused medical attention. On 12/13/16 reported no further injury, but asked for medical attention. Conclusion on 12/13/16 was not work related (repetitive motion with 9 months employment). Employee went for follow up on 12/20 when it was determined possible ligament damage and would possibly be work related.
Employee felt strain in abdomen when removing a broken lifter from the #1 dryer exchange. It became known on 11-29-16 that the employee was diagnosed with an inguinal hernia. No work restrictions recommended.
Employee stated was walking back from starting an auxiliary fan, stepped on a rock in a water puddle & it rolled out from under foot causing employee to twist left knee & ankle. Employee finished shift & reported but refused a medical evaluation. Prior to next shift the employee reported injury had worsened & requested to be seen by physician. Restrictions were issued.
Employee was walking on top of a dry bulk barge and missed a step, causing employee to fall. As employee was falling hit head and back on the dock. The result of this contact with the dock caused the employee to have Transverse Process fractures in lower spine and bruising to head and back.
Employee was climbing into the cab of a haul truck and hyper extended knee when the bottom rung of the cable step ladder swung forward. Employee reported the incident immediately and refused medical treatment. After no improvement in condition employee was taken to clinic on November 7th where EE was diagnosed with a knee sprain and referred to therapy for the injury.
Operating empty coal hauler traveling down haul road. Operator apparently dozed off, the hauler contacted a berm on the l side of the road, traveled the top of the 60' long berm, went off the l side of road, crossed a ditch and came to stop about 407' off the haul road. Operator received a cut to the back of head requiring sutures. EE stated was wearing seat belt.

EE was helping lift a box of floor sweeping compound when EE felt a strain in the right shoulder.

Miner was using a shovel to move material at the Hammer Mill Chute two to three feet so it could be accessed with a Dingo Compact Utility Loader. Miner hurt upper back during shoveling process.
Employee was prying a rock loose with a shovel, right hand slipped off shovel handle striking the edge of a steel beam, injuring ring finger knuckle.
The mechanic was placing a block under the service truck for repairs. As ee placed the block ee felt a pain in back. EE was seen by a doctor and released with no restrictions. On 10/21/2016 the employee went to the emergency room and was placed on work restrictions that could not be accommodated.
Before regular work shift started employee tripped and fell in parking lot requiring 5 stitches to right hand, 3 stitches to the right side of the head and a dislocated finger.
Employee stepped off cell and rolled their ankle on a guard rail.
Employee had pain in lower back after completing a screen change in the dry plant Rotex screeners. Employee was diagnosed with a slipped disc.

Another worker working across from IE was doing the same. one reached to grab a rock the other was going to break a rock and make smaller with a hammer. They both went for the same rock the EE with the hammer struck the left hand middle finger causing a smashed tip of finger.
Employee was placing steel bars on a shovel bucket. Employee cut one piece and picked it up, turned and placed it on the work platform for the bucket. Employee felt a pain. Employee was diagnosed 10/11/2016 with acute thoracic pain.
Employee was hooking up a portable heater when ee felt pain in left groin area. Employee was later diagnosed with a hernia. The employee had surgery to repair the hernia on 1/6/17.
Employee was carrying timbers in low height 46""-52"" over a distance of 200' over irregular bottom. Setting timbers and building cribs in a bleeder. Employee strained back. Employee did not start loss of time until January 3, 2017, and is off waiting for back surgery.

Employee reported the accident on 10-5-2016 but did not go to the doctor until 10/14/2016. EE said EE was climbing the ladder next to cab of the drill and felt a muscle in right arm pop and sharp pain. EE said EE had three points of contact and should have used EE's legs more.
Miner was in the process of erecting concrete block permanent stopping when rock became dislodged between roof bolts, fell, striking miner in the back and head area.

While an employee was installing a drive-line in a rubber tire dozer, EE felt a pop in EE's left bicep and experienced pain in the same arm. The diagnosis is a complete tendon tear.
Employee was installing a grease O-ring seal to a rock crusher. The O-ring slipped off from the back side where employee started. Employee fell forward to the edge of the brass socket liner.
The employee was lifting a Ram car drag into the bucket of the Scoop with another employee when the scoop bucket came down on the employees foot.
EE reported on 10/03/2016 that they felt some pain in abdominal area. EE said they felt a bump in May but it did not bother them until now. EE had a hernia the previous year and thinks they may have another one.

Injured employee (IE) and two others were pinning a feeder. During the work the IE states they felt a slight discomfort in the left abdomen. IE continued to work without incident. Three days later went to the emergency room because of pain, was told they had a hernia, and the doctor said stay home for one day. IE is on a 10 pound lifting restriction.
The miner was installing a Handrail on the new Rip Rap Skid. While trying to align the holes, the Rail shifted and smashed the right thumb. The incident resulted in a partial traumatic amputation of the Right Thumb
EE was cleaning the burner pipe and reported a sore back. EE was evaluated to have a sprained lumbar.

while employee was in the process of fixing a torn belt, employee placed one hand under belt and drilled through belt and made contact with gloved hand creating a circular injury to right pointer finger resulting in 8 stitches
The EE was operating an overhead crane and installing cone crusher steel. The employee lost their balance and fell down (on the same level) onto the crusher base cable swivel. The EE suffered fractured ribs as a result of the fall.
While descending a ladder off the pipe truck the employee stepped on a rock and hyper-extended left knee.
A miner was injured starting to drill a hole in the roof where adverse conditions existed. A piece of rock measuring 10"" X 16"" by 4"" thick fell from the roof and contacted the metacarpal glove on right hand. The rock was sharp and it cut through the glove. Employee received a laceration on two fingers on right hand.
Employee was pre-shifting lube truck. When employee exited the cab of the truck employee lost balance and fell to the ground. The fall resulted in a right shoulder contusion.
Miner was splitting a stone with hammer and chisel and struck finger with the hammer.
An adjustment needed done on the gate for the #8 limestone. This is usually performed with a 3/4"" ratchet and hammer. EE was using a sledge hammer which resulted in a mid-low back/front hip strain with right leg radiculopathy.
Injured employee was helping to demolish and old fence when a piece of angle iron fell striking employee on the upper right arm. The arm was not broken but there was a soft tissue injury. The employee has returned to work on restricted duty as of 10/28/2016.
The employee reported to EE's supervisor that EE was feeling a dull pain in EE's lower left back and around EE's left elbow area while EE was performing EE's regular job duties of sewing and stacking 50 lb bags of salt.

EE was performing pre-shift examination of EE's 992C front end loader and while walking around the backside of the loader EE stepped on a rock that caused EE to loose EE's balance. EE twisted EE's knee and heard EE's knee pop. EE was taken for medical evaluation and placed on modified duty.
Employee was watching the C/M machine cable as the C/M machine was advancing to another entry on the 17 HG section. EE had EE's back turned away from the roof bolter cable which was laying on the ground. When the roof bolter operator engaged the cable reel on the roof bolting machine the cable struck the employee on the back of EE's feet and caused EE to fall on EE's back.
The injured was struck on the arm by a rock that fell from the mine roof between the ATRS and the last row of bolts.

Employee was using a walk behind bobcat to clean up spillage under conveyor structure. After filling bucket employee lifted bucket with controls, causing the back end of the machine to rise pinching left hand between the controls and the conveyor structure. Pinching caused laceration and abrasion.
Employee was lifting a bucket containing an aggregate product sample for QC testing onto pick-up truck bed. While lifting, EE incurred a thoracic strain, resulting in restricted work duty.
EE felt pain in low back area while swinging a hammer to drive out a swing cylinder pin. EE placed on restricted duty for muscle strain and was released on 11.1.16.

Employee was assisting mechanic putting chain back together when mechanic struck piece of roller chain and a piece of metal struck employee in the right arm. As of 11/09/2016 restricted duty sutures required.
Employee states while employee was installing roof bolts in the Main North 2 travelway the roof bolter machine was bumped by a ram car causing injury to right hip. Restricted Duty.
Employee was laying on stomach in an awkward position, using two spud wrenches trying to free a steel frame from a wedged position. The employee felt a sharp pain in right shoulder.
Employee was descending the stairs on a haul unit when EE lost footing on a slick surface. EE grabbed the handrail with left arm and managed to swing self to the ground landing on feet. Pain in back developed over the next few days. Medical visit on 10/27/16.
Employee was investigating a needed repair to a headlight on a tractor when the engine cover shifted causing a laceration to left index finger requiring sutures to close.
Employee was flex lancing a vertical 6 inch pipe from the bottom. The opening was approximately 12 inches from the ground. As EE was flex lancing a piece of the product came lose falling and pulling the flex lance out. The tip of the lance struck EE's left leg below the knee causing a laceration. several times for follow up all ending in EE being released to light duty.
Removing a flat tire from a haul truck. The tire moved and caught employee's finger.
While unloading delivery truck, employee pulled on a pallet strap to reposition it. The strap broke, causing the employee to stumble backwards, falling off the rear of the van to the ground below. Employee returned to work but later complained of back pain. EE was examined by a physician on 11/3/16 and placed on restricted duty due to back pain and sprained ligament in back.
Employee was inspecting conveyors belts, found a froze idler, found a bar and tapped the frame of the idler several times and the bar slipped after several attempt, catching the tip of the bar on the next roller downstream, approx. 18"" away, started to pull the bar in and the bar was forced upward into the frame which smashed the employees thumb between the bar and the frame.
Employee was feeding a gravel plant with a Cat loader. There is a heater fan in the cab of the loader. The employee said the fan was blowing cold air on EE. EE tried to stuff a rag into the fan. Apparently EE's hand went into the fan, and EE cut the second and third digits of EE's right hand.

An employee was driving when EE ran over a pot hole causing EE to bounce in EE's seat and strike EE's chest against the steering wheel.
While installing a classifier on the wet site of plant, employee was welding on a new sand chute. At break time, the employee was descending the chute and foot slipped causing EE to fall on back. Employee seemed okay but was taken to the clinic to be checked out. Employee was put on light duty 10/16 and returned to full duty 10/18.
While employee was doing clean-up work using a shovel, ee reported ee injured back. Medical attention at local hospital.
Ee was lifting filter out of tub, leaned in and later felt pain in chest and abdomen area. Was not sure related to work. Sought treatment on 10/24/16, diagnosed with contusion, returned to work on restricted duty. After investigation by site and workers' comp, determined compensable on 11/14/16.
Employee was helping a co-worker work on a tail pulley. Employee was turning a threaded rod while a co-worker was hitting a bearing with hammer, bearing came loose suddenly and the treaded rod took skin off EE's fingers.
EE was descending from haul truck. EE continued to try and descend with lunch box in one hand and the seat cushion in the other, while holding handrail. EE slipped on the last step and fell approximately 30"" to the ground, landing on back side. EE didn't wish to see a doctor until the following Tuesday (10/18) EE was put on light duty and went on vacation effective 10/19/16.
Employee was climbing up dozer to fuel the machine, as ee grabbed the grab bar ee right hand, ee left foot slipped causing ee to strain left shoulder.
EE was lifting a new rock shelf insert, weighing approx 80 lbs, from baskets. EE raised it approximately 4-5 inches above the basket when the insert began to slip from EE's grip. EE's left index finger was caught between the top of the basket and the bottom of the insert.

A tube winder operator was poking some stuck cans near a roller when the roller pulled the poke rod under the roller. The operator instinctively tried to grab the rod but missed. EE's hand was pulled into the roller and the machine was stopped and EE got EE's hand out. The hand was x-rayed and no fracture was found. No stitches were indicated.
Elliott employee was disassembling a machine for repairs. A piece that was believed to be secured unexpectedly moved. Employees right hand was pinched between part and stationary object resulting in a laceration to palm of hand at base of index finger. Cut required nine stitches to close.
Employee was working on cleaning out lime from the #2 lime tank, using a pressurized water hose, also was opening and closing a 6 inch gate valve. They said that their shoulder got a pain in it that evening and they just thought it was arthritis, but the pain was still there after a couple of days. So they thought they should report it.
Employee was standing idler up under belt with a pry bar. The bar slipped and the employee struck hand on the plant support structure.
Employee was dislodging a jam in a rock feeder when the pry bar being used shifted, causing hand to strike the surge tunnel wall.
Employee was installing a hoist cylinder on a 2006 Volvo Dump Truck. The hoist cylinder moved and pinched middle finger on right hand. EE's finger had a laceration requiring four sutures.
Employee was standing on mud pan and stepped off and strained back. Mud pan is approximately 18"" off ground.
Miner suffered a strain neck when ee was struck on the top of hard hat by a lump of dirt that fell from the top of an excavator bucket that was resting on the ground about to be greased. The injury was treated as first aid and after a week the injury did not get better. Miner sought medical treatment on 10/19/2016 and was placed on restricted duty starting 10/20/2016.
Two employees were trying to lock down boomer with pipe handle when pipe come off causing handle of boomer to fly up striking one employee in the mouth resulting in a minor injury.
The employee was lifting side castings up and handing them to another employee. During one lift, employee turned to the right, while holding the side castings, and felt pain in right knee.
EE was operating a wheeled loader when EE encountered a bump in the road and states EE bottomed out seat causing pain to lower back and neck.
Employee was walking up stairs, reached 5th step and felt pop in right knee. Employee treated with ice pack and taken to clinic for evaluation.
Employee was going down 2 steps, rolled foot/ankle and suffered a fracture to 5th metatarsal.
While using a pry bar assisting in adjusting a pinion on a dryer the bar slipped and pinched the tip of the right hand ring finger. This action caused a nondisplaced distal phalanx fracture
Pressure was applied to roller mill hydraulic system to check for leaks. A leak was detected at fitting, which was tightened to stop leak. The fitting broke loose while tightening, and struck EE in the ribs. Physician diagnosed as contusion, prescribed limited lifting etc.
Turning of drive belts. Right middle finger pinched between motor pulley and drive belts. EE jerked EE's hand quickly which severed the tip of finger. No bone loss. Y flap to cover wound.
An aggregate laborer was attempting to loosen a cap on a fuel tank with a pipe wrench when felt pain in back. The laborer finished out the day doing normal duties and returned to work the next day and went about normal duties. At the end of the second day employee decided to seek medical treatment and was put on restricted duties for lower back pain.
An employee was pulling a wheelbarrow and felt a pop in knee.
The employee was covering the end loader during the operators lunch and jarred their back. Employee went to the Chiropractor and was given light duty.
Employee was assisting a contractor in a 54"" Conveyor belt replacement on the RP1 Radial Portable Conveyor when right middle finger was injured. The group was preparing to lift three 54"" troughing idlers with the 75 ton Linkbelt crane when the employees finger was caught between the grab chain and an idler frame.
October 10th Employee was using two wrenches to tighten DBH1 damper cylinder bolts when the lower wrench employee was holding with left hand slipped and employee's left wrist bent and hit the cylinder. On October 12th employee was taken to Urgent care doctor said employee had a hair line fracture to wrist.

Employee was troubleshooting a conveyor when a rock bounced off and struck employee in the top of the hard hat. As a result the employee is experiencing neck soreness and headaches.

EE was shoveling in P-9 tunnel and felt a little pain in right shoulder on Saturday and thought nothing of it. That night EE woke to considerably a lot more pain and the next morning EE notified supervisor. On Monday the 10th EE requested to go to the clinic and EE was provided that.
Employee was putting exhaust back in the 988 loader. Went down steps to other side of loader and pulled muscle in back.
employee was changing lights on #3 RK Kiln Deck. As EE was helping to dismantle the scaffolding, EE's left foot slipped into a groove twisting EE's left ankle. Pain and discomfort increased and subsided over the next few weeks. Ultimately, the pain and soreness increased prompting a medical visit on 11/3/16.
Removing bearing from loader frame. Track bar skipped off the bearing sidewall and into the bearing bore. Caught technician's left pinky finger between the track bar and loader frame. Partial amputation of pinky finger tip.
Employee was exiting from skid steer when employee rolled ankle resulting in a sprained ankle.
Blaster accessing top of truck on ladder when ladder separated from truck body causing Blaster to fall against cab of truck, resulting in a left shoulder strain.
EE got thumb on right hand stuck between a box of powder and the railing on the platform. EE placed on restricted duty.
Employee mis-stepped as EE was crossing over a beam. EE's foot came down partially on the ground and partially on an access door, twisting ankle.
The employee was shutting the door on the haul truck and got middle finger on left hand between the door and the door frame. This caused a laceration to middle finger.
The employee was running a loader and as it was going down the hill into the pit the bucket's edge starting digging into ground making it come to an abrupt stop.
EE WAS TRIMMING A PIECE OF SHEET STEEL SO IT WAS EASIER TO HANDLE. THIS PIECE WAS 1/4""X 2'X 4' EE WAS HANDLING THE STEEL WHEN IT SLIPPED OUT OF HAND AND LANDED ON LEFT FOOT RIGHT BEHIND STEEL TOE. Originally this was a 1st aid however due to the swelling not going down ee was checked out an via an x-ray 10/17/16 it was determined ee fractured second metatarsal.
The EE was operating a R-65 Haul truck and smelled rotten eggs, so rolled up the windows. A little later the battery exploded and ruptured EE's right ear drum. Upon further investigation, the alternator was over charging the battery which caused the explosion. The EE was restricted for five days.
Employee strained right shoulder while using a 3 foot pipe wrench to remove a 4 inch ball valve in the road rock tunnel.
Employee was working in the tunnel around the surge feeder on the platform and had left the trap door open. EE was spraying down the feeder with water when EE had sprayed self in the face and stepped back into the access point falling less than 6' onto the dirt below. An employee was operating the #544 Haul Truck. As the haul truck was being loaded by the #268 loader at the 72 stockpile, the employee was jarred in ee seat as the first load was loaded into the truck box. After being loaded, the truck proceeded to dump at the crusher and then traveled back into the west pit where the employee started experiencing back pain.
I was checking on finding some Anti Freeze and I stepped off of a pallet onto the oil pit storage grating. The grating wasn't laying down all the way and it flipped and I fell into the oil pit. Please note:Employee did not report the accident until 11/28/2016
Miner was using pry bar to free up a chain on a sprocket. When miner pulled back on bar it slipped off the sprocket, hit a steel plate pinching finger between bar and plate.
Employee was hitting a belt clip with a chisel when a piece of steel broke off from the clip and entered the employee's left hand causing a puncture wound.

An employee was harvesting, noticed a low hanging edge strip so flew the load to the bottom of the back bay. Then used a hand tool to lift one side of the blank off when felt a sharp pain on the right side of back.
The employee was removing a snowman, when a built-up fell and hit the lance of the water pressure gun. This caused that EE's left hand turns. EE felt a little pain, but EE thought it was nothing. On October 8, 2016, the employee mentioned to EE's supervisor what happened, but told supervisor that it was nothing. On October 24, 2016, EE's supervisor sent EE to a doctor.
Employee was in the process of dismounting a light vehicle tire off of a wheel, using tire bars when employee felt a pop in right wrist. Employee went to the doctor and was placed on restricted duty.
EE hit hand while opening a kiln door. EE had hand evaluated on 11-10-16 and was put on modified duty.
Employee was operating a hand flow gate in boat loading tunnel. Gate was stiff and employee felt a sharp pain in left shoulder.
An equipment operator was operating a 40 ton haul truck when EE encountered a pothole in the road jarring back.
The injured was using a slate bar to pry a shuttle car tire from the wheel's stud bolts. The slate bar slipped causing hand to strike against the frame of the shuttle car which resulted in a fracture to thumb.
While spraying 3rd floor tower north side, middle door, a piece of hot material spit back onto employee's pants, just above boot line burning through jeans, sock, and lodging inside boot burning left ankle.
Employee was moving 50 lbs bagged product and experienced pain in lower back.
Employee was climbing the ladder of the 972H front end loader and lost grip with right hand and while do so left pant leg caught on ladder causing the employee to fall.
Employee was chiseling stone and ee felt foreign material enter eye.

Employee notified safety on 12/5/16 that EE bumped knee against a bench in the bath house on 10/31/16 while walking to EE's locker after showering after EE's shift. EE finally saw a doctor in March 2017 and was prescribed PT in July of 2017; EE's first session was July 13, 2017. This has recently been accepted as work related for patellar tendonitis.
Coming down ladder of haul truck to step on ground an felt ""tear"" in abdomen at location of previous non work related surgery.
While working on #18 Cement Silo to ascertain the nature of a leak from behind the dust collector housing to get a closer look at the problem. As EE looked around the DC housing, approximately 2 to 3 feet from the leak, the wind changed direction and blew cement into face and eyes.
Employee was torch cutting a roller free and the roller end dropped onto employees fingertip. This resulted in a fracture to tuft of finger and a laceration.
Employee was opening the shop doors when one of them slipped and cut the palm of hand, stitches required.

Employee was removing banding from a lift of roof screen, while removing banding the band sprang back resulting in a laceration to EE's arm causing employee to receive three stitches. Cause of the accident was lack of concentration. PPE was not a factor, mining equipment and mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.
Wearing utility knife on chain hanger off belt, beside radio. Knife safety latch hit radio and extended blade, unknowingly. Miner went to pick up ventilation tube and ran left forearm across the open knife blade resulting in a laceration that required 3 sutures to close. No problems with PPE, mining methods, training or compliance issues.
During the clinker sample, the wind was blowing and an air cannon went off causing clinker material to be blown around. Debris got past my safety glasses and side shields and into my left eye.
Employee was turning a valve and twisted EE's back. EE reported the incident 10/28/2016 however did not seek medical attention until 11/03/2016 when EE was diagnosed with a lumber sprain.
Mechanic was handling steel grating when a piece fell and mashed EE's finger causing a laceration. EE was taken to a local ER where seven stitches were used to close the wound. It was later discovered that EE also had a fracture in the tip of the finger.

Injured was rotating driven pulley by hand in order to distribute lubricant when EE's finger became pinched between the rpm sensor and the rpm pickup point.
Employee was using a grinder for demolition, grinder kicked back and cut employee's right hand.
Miner was washing haul truck while standing on the trucks access ladder. As employee opened the spray nozzle, the pressure caused the miner to become unstable and fall from the ladder approx. 4-5 feet. Employee fell onto left hand causing a fracture to left middle finger.
Crawler Mobile Tailpiece (CMT) cats were stuck and EE was attempting to free them with assistance of 3 ton chain hoist. The tail hook slipped off the CMT cats and flew off under tension, striking EE in the upper lip, resulting in a laceration. Received 23 sutures. No problems with mining methods, training, PPE or compliance issues.
EE was helping push a rock manually with a rock bar at this time another EE started to chop a rock at this time a small rock broke off of the rock that was being chopped and came back and struck EE in the right Hand and middle and ring finger causing EE to have a small cut on EE's fingers.
Employee was chipping the slag off a weld with a hammer. A piece of slag flew into left eye. The employee was wearing prescription safety glasses with side shields. The employee was working over the weld and the slag flew up and underneath the glasses. There was no damage to the eye. Antibiotic eye drops were prescribed for prevention.
While repairing the safety Latch that holds trailers in place at the dock bay the shock came off unexpectedly causing it to hit EE's right index finger and thumb, resulting in a cut that needed stitches.

Injured EE was tying a piece of rebar back into place after moving it to clear a concrete wall form. As EE was pulling the tie wire tight, the pliers slipped free, striking EE in the upper right side of mouth/face. This resulted in the upper, second tooth to the right from center, tooth being chipped and loosened.
EE was loading the tray for the Roof Ranger II roof bolter with roof bolts. While pulling a roof bolt off the side of the roof bolter, EE lost footing, causing the bolt being pulled on to strike EE in the mouth. The impact from the roof bolt caused one of the front teeth to break (fracture).

Injured was installing Ziggy rails and jacks on the S4D belt line and lacerated pinky finger on right hand. The injury required 5 stitches.
Employee sustained laceration to right middle finger handling a warming oven. The employee was assisted by a partner in lowering the warming oven to the floor when the finger was pinched between the oven underside and floor. Employee was seen by site responders, and then brought to hospital where medical treatment was provided.
Employee was cutting a couple of inches off slicker pants, as employee cut with the blade coming towards body, knife slipped and cut left thigh.
Miner was tightening a hose fitting when wrench slipped causing miner to lunge forward and hit head on hose fitting.
Employee was putting in a top roller while moving belt when EE noticed that the belt clamp was stuck. When the employee reached down to free the belt clamp another employee grabbed the roller and caught EE's finger between the top rail and the belt roller smashing and cutting ring finger on right hand.
Employee was grinding off old welds on a conveyor drive pulley to replace pulley lagging when a piece of metal went pass EE's safety glasses and face shield and entered EE's right eye
Employee was performing maintenance on the carrier bearing of a screw conveyor. EE slipped and struck EE's hand on the sharp corner of a nearby tool box resulting in a laceration on the side of the palm of EE's hand. EE received 6-stitches in EE's hand and was immediately released to full duty.
I.E. was taking material to roof bolter in #1 entry. EE parked & shut the scoop EE was operating off. EE got out of the scoop & walked between the scoop & the rib & that is where a piece of hog panel that had been trimmed off snagged EE's left ear & caused a laceration. Employee returned to work on EE's next shift & missed 0 days.
Employee was doing repair work to oversize chute at the plant when ee went to lift a 10x2 piece of channel iron and it slipped from right hand and fell on left hand smashing left ring finger.
Employee was attempting To cut a plastic wire tie to remove an old hose from the slope belt. The employee slipped and cut the back of EE's left hand and received 10 sutures. This is a NDL accident.

While using an angle grinder, the disc came apart and struck the user in the chin area. User was grinding on metal on a conveyor when the incident occurred. User had recently replaced the disc and failed to put the guard back on. Also contributing to the injury is the fact that a face shield was not in use. User was taken to hospital and received medical treatment.
Employee was closing the freight elevator door and accidentally got one hand between the doors and closed the door on fingers causing a laceration to right pointer finger.
Employee was hanging a piece of tin when a another piece of tin came loose and struck right wrist.(Medical treatment sutures required) Compliance with rules and regulations was not a factor.

Individual welded majority of Monday with no signs of injury and completed full shift. Early Tuesday morning, individuals eyes were burning. Went to doctor and confirmed individual burnt cornea from welding. Eyes were more sensitive due to lasik surgery previously had and was unaware long periods of time welding would effect EE. Doctor prescribed prescription eye drops.
Employee was getting a piece of rock off of the continuous miner and when ee tried to flip the rock in the pan it caught right ring finger and right pinky finger on right hand causing fractures to the tips of both. Note: Employee returned to work on the next regular scheduled shift.
Mechanic got fingers on right hand cut by the heater blower fan on a 992 loader
The EE stated that while installing belt structure, a belt cradle slipped from EE's hands striking EE's left index finger resulting in a laceration. The accident occurred because the EE failed to maintain a firm grip on the cradle. EE was wearing the appropriate PPE. EE is properly trained, has a good safety attitude. EE did not violate any rules, regulations or policies.
Employee of Overley's, MSHA Contractor ID L576, was cleaning out a sump with a shovel when right leg contacted a piece of corroded rebar causing a laceration which required stitches. Employee returned to full duty after ER visit.
