A roof fall was discovered at crosscut 16-11 between the track and belt. The fall is approximately 8' high, 15' long and 15' wide. No ventilation or travel has been affected. The fall will not be cleaned.

An unplanned roof fall occurred in the #14 Cross Cut of the #1 Section's Left Return. The fall was approx. 50' in length, 20' in width, and 7' in height.
North skip dump wheel broke off due to fatigue causing the skip to not be able to exit the dump scrolls.
On 12/28/19 at approximately 10:15 am, an elevator guide roller adjustment bolt was found broken on the Camp Run Elevator during inspection. The elevator was removed from service and the guide roller and adjustment bolt were replaced. The elevator was returned to service at approximately 1:30pm. There were no injuries or entrapments.
The mine had elevated levels of methane which called for an evacuation. MSHA 800 number was called out of courtesy even though it was not required under part 50. Because of the call we are submitting this 7000-1.
An unintentional roof fall occurred in 7N Mains at 44 Wall #6 Entry. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer in the mine roof.
Found an unintentional roof fall in the #3 entry between breaks 19 and 20 on the #12 Belt line. Fall dimensions are approximately 20' wide by 60' long by 3' thick.
During a pre-shift exam following an idle shift, an examiner discovered a rock fall in the South Mains travelway at xc73. The examiner called out to the mine manager who investigated the fall and determined it was a reportable roof fall.
an alarm for high temp came up on kiln 2 shell scanner. The supervisor on duty immediately inspected the kiln and found that a fire had ignited on K2 drive pier on the main drive gear.
Rockfall Occurred between breaks #67 & #68 of the 1 West Mains
During the examination of the Hanson hoist rope, there was damaged strains discovered that surpassed the allowable limit. The hoist was shut down and miners were withdrawn from the affected area. The Hanson hoist rope was replaced.
Smoke was observed coming from areas of the raw coal pile located near the Prairie Eagle East Portal. Actions are being taken to remove affected material from area.
The small two-man hoist on the Camp Creek portal site went down due to a bad VFD control card during a daily check.
Coal fire (smoldering) in the coal yard. HEO notified management. Management called MSHA online. Have been working on extinguishing fire this past week.
Found an unintentional roof fall in the #4 entry between breaks numbers 5 and 6 on the #4 Belt Line. Fall is approximately 20' wide by 60' long by 4'-6'thick.
The Elevator for the Sparta Portal was down due to the Carlock Door Switch was out of alignment.
Smoke was observed on the west side of the raw coal pile. We have continued our efforts to extinguish all known hot spots using dozers and an excavator.
4 persons were entrapped due to an electrical fault at Sparta Elevator, stopping unexpectedly 10' from the top landing. They were entrapped for approximately 30 minutes with no injuries. The Mechanics had to reset the power and the elevator continued to the surface. Once at the top landing the persons exited the elevator and the door sensors cleaned as a precaution.

An unintentional roof fall occurred in the G1 Headgate longwall section located at 33 wall in the number one entry. The fall measured approximately 6'high X 15'long X 8'width. This was due to a sagging roof, Stage loader caught roof strap causing material to fall.
A roof fall occurred at XC#75 MS#13 belt line. The belt had been removed and was under planned rehabilitation. Personnel had withdrawn from the work site. There was no interruption to production. There was no injury or property damage with this occurrence. The area has been cleaned and bolted according to plan.
Driver was possibly distracted and backed into an occupied haul truck that was staged and being loaded with shell rock.
Foreman was conducting workplace examination of the mine. Foreman saw the roof fall when foreman entered into Main 10 from Main 9. The roof fall is in by of crosscut 10. The exact date of the roof fall is unknown because the mine was unoccupied on 12/14/2019 and 12/15/2019. MSHA was notified within 15 minutes of the fall being discovered.
An insulator on a 138kv electrical power pole went to ground and caused power outage and a unplanned fire on the pole at the north end of the plant. The fire burned 67 minutes until FD put it out. The pole is owned by the utility provider, not the mine.
Smoke was observed coming from the northeast side of the raw coal pile located near Prairie Eagle East Portal. Actions were taken to treat and remove the affected material.
Operator was cleaning out from around the 89 conveyor when EE accidently hit the conveyor with the loader. There was no injuries.
Smoke was observed coming from the south side of the raw coal pile at Prairie Eagle Plant. Actions were taken to treat and remove the affected material.
The North Portal Hoist went down with an electrical issue. A blown fuse was found and replaced. It was then found the hoist was not reading the brake pressure to release the brake. Pressure switch was changed so the hoist read the pressure and it was test ran 20 trips with non issue.
At approximately 4:45 a.m. an employee was traveling in the right return near #2 belt head when EE encountered elevated methane levels. All power was de-energized in the affected area and the entire mine was evacuated and both agencies were notified. No one was trapped or injured.
Coal was observed to start smoldering on the coal pad. We immediately began looking for the source. We began digging the large pile and compacting it to smother any smoldering. It took approx. 8hrs to dig out the source that was centrally located at the base of the pile. Called in as soon as we were confident there was no smoldering
Stove in break room of shop caught fire resulting in total loss of shop building. No injuries.
On Friday Dec. 13th the entire plant lost electrical power at 11:01am. There were two employees in the old raw mill elevator when the power went out. They became entrapped in the elevator between the 3rd and 4th floor. Radio contact was maintained & both employees were in good health. Plant regained power at 11:37 and elevator went to landing & employees got out.
There was an abrupt motion inside the car caused by one of the miners while traveling down that activated motion switch that caused the elevator to stop therefore causing the miners to be trapped. Elevator service co. responded and miners were released within 58 min. The elev. was inspected and returned to service.
Two maintenance employees and a designated fire watch were tasked with patch welding holes on the Rougher Tail Discharge Box A&B. After realizing the hole was larger than anticipated, they left the area to get a larger patch for the hole. They proceeded to weld a small portion of the hole located on a rubber lined pipe and it caught fire.
Skip kicked out at 900ft breaker tripped. Wire that was not in use shorted out air pressure switch. Wire unhooked, capped off. Ran test trips and skip was back online at 3:00pm. No one was on the skip at the time.
At 4 AM on 12/12 a coal fire was noticed in the coal yard. MSHA was contacted. The HEO used the excavator to mitigate the smoldering. The fire was out by 2 PM on 12/13.

Employee was trapped in the Step 1-2 Crusher elevator around 12:15pm. The elevator was stuck just below the 3rd floor. MSHA was notified at approximately 12:40pm. The elevator company was called and the employee was freed at 12:53pm The elevator was put back in service.

A roof pot out occurred at cross cut 120 in the number 1 entry of tailgate 2. The fallen roof material impeded passage through the area.
A roof fall occurred along MW#1 belt, XC #31/station 35+45, entry #0. Limestone had 2 slips that contained quartz, 1 in the middle and 1 larger slip in the heavy side of the cantilever. There was no injury or property damage with this occurrence. The fall was approximately length 18' X height 11'.
An unintentional roof fall occurred in the No. 8 entry on the North Mains section. The fall occurred as the Continuous Miner operator was backing the miner out of the last lift. Material fell from the lift just completed and extended into the entry, pulling bolts and catching the miner head. No Injuries were suffered from this event.
A fire started at the Screening Tower at Quarry. First responders were called immediately. The fire burned for approx. 90-120 minutes. Investigation and repairs are ongoing. Damage is currently unknown.
An unintentional roof fall has occurred on the working section the fall was located at the 20th XC of the S4I between the entries of 9 and 10. The fall was approximately 30 feet by 17.6 feet by 6.9 feet high.

Skip parked at landing (estop engaged), hydraulic pumps started. The hoist operator saw that faults came up & wouldn't clear. We troubleshot, but no fix. Called our elec. eng. & manufacturer. Manufacturer ran troubleshooting but wouldn't reset. Called MSHA at 615pm, started evacuation. Manufacturer got online & figured out what breaker was tripped, we reset, ran test trips, fixed.
A small coal fire flared up due to combustion and had not been buried within the 30 minute time frame in the south east corner of the Canyon seam.

Roof fall occurred on belt line due to slicken sided undetected roof. Top was also bolted with approved 4' bolts.
An unintentional roof fall has occurred just outby spad 7022, measuring 15' X 8'. In addition, roof conditions have deteriorated and affected travel in the Primary escapeway that runs through the Mains and 5A sub-mains panel.
The lilly controller needed adjusted and the hoist was removed from service.
Spontaneous Combustion Fire (smoker) was found in Area 0 at 9:00 am. Called in at 9:16 am. The smoker will take more than 30 minutes to bury with dirt. D-11 actively working to bury smoker.

An unplanned roof fall was discovered in the 6 North #2 Entry at 18 block. The fall was approximately 16' wide, 30' long and 9' high. The fall was most likely caused due to gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.
Found an unintentional roof fall while conducting a pre-shift examination. The fall is located #6 entry, South Mains, 3 breaks from seal #5 of Seal Set # 30.
A fully trained miner was fatally injured while attempting to remove a splice pin from a 72"" mechanical belt splice. The splice was positioned near the outby side of the tail frame. The victim was positioned between the bottom conveyor belt and the top conveyor belt. Investigation is on-going by company, state and federal officials.
A contractor was servicing the Alimak elevator at the fourth level of the pre-heater tower. The contractor was on top of the elevator and a mine employee was inside the elevator assisting. The person inside the elevator heard a clicking sound and the elevator started a downward descent. The contractor on top of the elevator received crushing injuries when the elevator moved.
Maintenance employee was changing brush on mobile sweeper. While picking up the brush they strained and felt some pain in groin area. This straining caused a hernia on the right side of groin area.
Employee's right index, middle and ring finger were caught in a rotary feeder.
The employee stated was pulling the drill steel out of the rib and accidentally bumped a control causing left pointer finger to be pinched between the drill chuck and drill steel. This resulted in a partial amputation of the left pointer finger (first knuckle from tip).
EMPLOYEE CAUGHT RIGHT HAND MIDDLE, RING AND PINKY FINGERS IN AIR ACTUATED KNIFE GATE VALVE ALMOST SEVERING THEM REQUIRING SURGERY.
While working on the front end of a haul truck, employee was lifting a lock ring and felt a pain in their lower stomach. Employee set the ring down and felt a small bubble on lower stomach.
Guiding the rope during stripping covers off for the vulcanized splice, resulting in amputation of the pinky finger tip.
Ee was seated behind front passenger seat in mantrip. Ee was closing the compartment door to the right from a seated position, and placed right hand in the door frame near the hinges. Ee kicked the door shut, pinching right middle finger in the door. Amputation of the tip of the right middle finger.
Employee claims that while adjusting a scrapper on a belt conveyor, the two right middle fingers were pinched in the belt.
EE was stacking 50# sand bags on a pallet. When EE turned to place a bag on the pallet had a sharp pain in the abdomen. Later in the day EE had a lump protruding from the same area. EE can push the lump back in. *On 03/11/2020 EE saw a doctor & was diagnosed with a hernia. EE plans to have the hernia surgically repaired. The doctor issued a Return to Work slip with No Limitations.
Belt was turned off. Rock was stuck under belt. EE was using a bar to hit rock to try to knock thru the other side. Rock broke loose & EE's right hand got stuck under belt. A co-worker had to cut the belt to get EE's hand loose. EE was able to move all fingers but was really sore. Went to hospital to have x-rays to check for broken bones. Had swelling & bruising.
Employee was getting down off of the LW shearer when they slipped and fell striking left knee. The employee worked till 1/20/2020 when they were taken off work.
EE was climbing down a step ladder and turned to put EE's foot on ground and felt EE's knee pop.
A haul truck was traveling down from Pushback 61 ramp & lost traction. The haul truck began to fishtail & it clipped another haul truck traveling up the ramp loaded. The EE felt soreness and tightness in back the next day when they woke up. EE was taken to the clinic & issued restrictions.
A fully trained miner was pulling a cable when the miner experienced pain in lower back. The injury was not the result of any compliance issues nor were any protective guards involved.
Employees were working on the J50 Portable Jaw Crusher. The plant was down and employee was helping the mechanics repair the radiator. Employee was standing on the hydraulic tank handing the mechanics the parts. Employee then walked onto a ledge (6-8"") to get down, lost footing and fell approx. 10-11 feet.
Employee was scaling the footwall rib & when employee was pushing on the bar to remove a piece of loose & the loose popped off the bar reverberated causing a strain to employee's right wrist. Seen by site medic, referred to ER was given full release but next day employee was still having pain in wrist and was restricted by employee's physician.
The employee was climbing up on the rear bumper of the miner to repair an oil leak and slipped off and struck EE's right knee on the bumper. The employee worked till 1/02/2020 when EE was taken off of work.
While employee was conducting a screen change employee came in contact with sharp edge of screen resulting in a laceration that required 20 stitches
Employee was starting to drill a new hole when the steel hit them on the back of hand.
IE was operating crew van, contacted berm with left side tires, the impact caused a compression fracture in the lower back
The injured employee was bolting on the return side of the miner on the 1WTG Section. The miner pulled up catching the ventilation tubes causing the slider tube and the next tube back to fall striking the employee in the back. No laws or company polices were violated. All appropriate PPE was worn.
The sample crusher was plugged from the crusher to the head of the primary belt. The employee used a scraper to try to unplug crusher while it was still operating. The crusher segment hit the scraper causing it to come upward striking EE in the mouth and cutting EE's hand, resulting in laceration.
Employee was descending the lower stairs on the haul truck and fell to the ground landing on their left knee.
Employee pulled hamstring while walking around truck during pre-op resulting in one day of lost time.
Operator tramming track dozer 26641, encountered a rock caused operator to inadvertently set park brake. Machine suddenly stopped, operators left shoulder jammed causing a shoulder strain. Employee was given a work as tolerated release 1/2/2020, then on 1/9/2020 notified the mine that the MRI showed rotator cuff tear, taken off work 1/9/2020. Begin physical therapy 1/13/2020.
Ee was cleaning an area and lifting a wheel barrow full of rocks when EE felt pain in EE's abdominal area. EE was diagnosed with an inguinal hernia and referred to a surgeon. EE was released to restricted duty.
EE's boot caught on L63's boarding step while boarding the loader causing EE to lose balance. EE felt discomfort in right forearm that was holding the handrail. Evaluated in the ER and taken off work.
Employee was standing on the 3rd rung of an 8' step ladder, operating a chain come along, lowering a chute from a storage bin. When the chute was approximately half way between the bin and the floor, it shifted and knocked the employee off the ladder. The employee suffered a fractured pelvis.
The Employee and two others were lowering a feed box to the basement floor. The feed box was setting on its edge, while there was slack in the hoist rope the employee bent over to unhook the load when it shifted causing the hook/sheave to hit them in the head. Employee went to the Dr. on 1/17/2020 and began physical therapy on 1/20/2020.
Employee was hanging the miner cable when employee felt pain in left elbow. Employee continued working until 4/10/2020 when employee was taken off work for surgery on left elbow.
Washing a van at the mine wash rack, while spraying the van, employee stepped backwards and tripped over equipment wheel chock.
A fully trained miner was loading a jack when the jack rolled off the car onto EE and EE fell backward. The injury was not the result of any compliance issues nor were any protective guards involved. The employee did not begin missing work until 12/28/2019.
Employee was walking under #1 kiln by tunnel entrance when employee *said* employee slipped on ice. *NO ICE WAS FOUND!
Employee was side bolting along the continuous miner in the G2 Section #2 Entry, when a rock fell from the rib protector and struck employee in upper back/neck. Employee moved away from that area when a second rock struck employee on top of head. Employee was diagnosed with concussion like symptoms on 1/10/2020.
Miner slipped while walking and hit ground 12/20/2019 bruising elbow hip and knee, but has NOT received any medical treatment until June 25, 2020. Miner has now had outpatient surgery to scrape scar tissue off elbow.
Employee stated that EE was in the process of checking the section feeder and was walking around it when EE stepped into some muddy conditions and twisted EE's right knee.
Employee was climbing out of supply motor and while doing so felt a pull on left side hip. Employee was wearing all required PPE.
Injured employee was observing rail sections being drug along the longwall power car from the corner of an adjacent cross-cut. Chain was connected to a nylon rope being used to pull the rails with a winch. The connection between the chains & rope failed causing the chain to recoil and strike the employee's right leg causing contusions & lacerations.
Employee was working on the crusher. EE went to grab a tool when EE tripped over frozen mud. Tried to catch self on nearby truck, but fell anyways pushing EE's arm behind self.
Injured claims they strained their back when they reached over their head to get a curtain in place in the #8 entry. Injured was placed on restrictions for their strained muscle.
Employee attempted to do a spot weld without properly positioning welding helmet. EE suffered irritation to both eyes. EE was welding on a gravel trailer.
Driver was dumping a load of coal when the hoist on the trailer failed. The trailer fell quickly and did some damage to the trailer. The driver was inside the tractor and was shaken up.
Snowed night prior. LAW Inc Crew had plowed facility and was in the process of sanding. EE had been asked to wait till we had sanded and in the meantime fell on the slippery surface. 911 was called immediately.
Trying to pull door open that was frozen shut. This was at starting time and door had frozen shut during the night.
While carrying a jackhammer to the next room the fitting broke causing the hose to swing and hit them in the groin.
Employee experienced a torn tendon in their shoulder due to diagnosed severe tendinosis of the supraspinatus tendon and of the bicep.
EE was walking across uneven ground on mine site and slipped on ice and fell to ground landing on back.
EE approached 420 scaler to get the attention of the operator. Operator confirmed by hand signal that operator saw EE. EE came to the window to discuss moving scaler to the side to make room to survey. Operator & scaler prematurely moved the ""outrigger"" & it was sat on EE's rt foot. Operator immediately raised machine when operator became aware of the incident.
Employee was loading pin supplies from a scoop bucket on the ground to the roof bolter. In the process they strained their groin. Employee started missing work on 1/2/20.
Backing down from a dirt pad with drill, got too close to edge of the side of the pad and dirt gave away, drill rolled and turned over knocking out window. Operator hit head on the roof inside cab, then rolled out window landing on shoulder, neck, and back.
Employee was working with the blasting crew. They were on the bench waiting to move to the next hole to load. When it was time to move employee stepped back and stepped on a frozen rock, which rolled an ankle. Later after x-rays it was determined the ankle was broke.
At approximately 11:40am on 12/18/19, an accident occurred in which IE was caught between the Brookeville tractor and a roll of 72' belt. A group of miners were arranging to push a roll of belt onto a sled. As the roll belt was being pushed it caught the lip of the sled. The entire sled moved forward and a 2nd roll of belt pinned the IE against the tractor.
Worker has been under doctor's care since 12/23/2019 with no restrictions. On 01/03/2020 worker's doctor has now applied work restrictions. No bending, climbing, no operating heavy equipment, no lifting more than 25 lbs. Back related injury received when dozer allegedly ran into the 740 truck.
While traveling through the escapeway door, the employee's left leg got caught between the door and frame of the Fan House.
Employee was using a scoop to move a refuge chamber when the scoop operator side tire slipped off of a ledge, causing the scoop to come down hard, jarring the employee causing injury to EE's back.
Employee was crossing over a belt crossover, slipped and fell. EE's left foot got caught in the crossover ladder spraining the left ankle.
Employee was throwing a track switch and felt a sharp pain in the right shoulder. Employee continues to work but has started receiving physical therapy treatments on 1/21/2020.
EE was walking down roadway from job site trailer to the working area when EE slipped on ice and fell.
While walking on a slightly pitched roof, employee slipped and fell to the roof.
Employee was traveling out of the parking lot in a pickup truck beside a rock truck, when the rock truck made a right turn, hitting the front end of the pickup. It pushed the pickup truck to the right, jerking the employee around.
Another miner applying alcohol to truck airline as EE walked around the front of the truck, airline inadvertently disconnected & sprayed EE with alcohol. Some alcohol got into EE's eyes, eyes were promptly rinsed with eye wash. Miner was informed if irritation returned to go to emergency room. ER/Dr prescribed eye drops and advised against work for 3 days.
Employee was changing a light in the shop area of the Prep Plant when they tripped over a pipe lying in the floor and fell to the floor landing on left knee causing pain.
Employee had picked switch throw up with EE's hand and went to step on it to switch rail position. As EE pressed on the throw with EE's foot EE felt a sharp pain in EE's foot. Employee is trained and was wearing all required PPE.
Employee was moving a beam when they strained groin/abdomen area. Cause of this incident was lack of employee concentration. PPE was not a factor, mining equipment was not a factor, compliance with rules/regs was not a factor, miner proficiency may have been a factor.
Employee was walking across the Luxe Ready Line when EE tripped on a rock and clump of mud.
Employee was walking the beltline and discovered a piece of drawrock in the walkway, under the lifeline. EE bent over to flip the rock out of the walkway, and felt a pain in EE's lower back and left leg. An MRI indicated the employee had a herniated disc in the lower back.
Employee was using a CMAC to install a bolt when a finger was contacted by the drifter. Employee was attempting to line up drifter and driver. Employee was seen by Medic and referred to ED where restrictions were issued for ligament damage to finger.
On 12/14/19, employee was loading retail coal, pulled lever to close hopper bin, stood up from folding chair and felt pain in lower back area. Employee refused treatment. On 12/30/19, employee was still sore and taken to local Urgent Care for observation, and diagnosed with lower back strain.
A driver rolled ankle while making a delivery to the Bellingham plant. The rolled ankle resulted in a fracture.
An employee was attempting to free a stuck airlock door that had been damaged. While pulling back on the door, employee fell backwards on to left ankle resulting in a broken left leg.
Employee was making up split set bolts for partner to install, when turned to take a step they stepped on the corner of a bolt plate causing the bolt to spring at them contacting left cheek & lower left eyelid area causing a laceration that required sutures.
employee put a hand on the bolter and a piece of shale feel from the top and hit middle finger causing a small fracture.
Employee was helping hang a vent tube when a piece of rock fell out from between the roof screen and struck EE in the left wrist. The employee suffered a fractured wrist.
IE was climbing down a D11 dozer with dinner bucket in hand when they fell to the ground and fractured the distal end of right radius in wrist.
Employee stated EE was in the process of hanging fly pads in the #4 entry, on the Unit 2 Section. In the process of EE hanging a fly pad from the 6"" ladder, the ladder tipped over, and EE fell to the ground. This caused EE to land on EE's head, shoulder, and back. Note: EE was standing on the bottom 2nd step of the ladder when this incident occurred.
Folding discharge conveyor section 1 miner was going to attach chains to head pulley for lowering. Bolts were held in place by tension only when movement was applied bolts loosened/fell conveyor collapsed. Miner fell 7' +/- to ground landing on lower back/hip and hitting head also, brief unconsciousness.
A fully trained miner was prying a sand prop from under the longwall mule when the prop came free and smashed EE's finger between the mule and the prop. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee, (Continuous Miner Operator) was found, by the shuttle car operator, pinned between the right rib and the right side of the continuous miner. Employee was backing the miner out of the #2 heading after a 10' cut. During this process employee was securing slack in the miner cable by placing a pull strap on a bit and bit lug on the cutter drum.
The employee was going from the worksite to the plant with a load of coal when the driver came upon a rise in the road and they missed a gear at the top of the rise. The truck was then in neutral and they lost control on the down grade of the rise and the truck turned on its side. The gear shift struck the employee in the rib cage causing a possible fracture to rib.
Warehouse employee was relocating three 55 gallon drums in back of bed of truck to balance load. The drums of oil and DEF were being delivered to customers outside of mine. Initial injury appears to be a strained back.
Miner slipped on snow or ice while walking in the quarry. Back pain at time of incident.
Employee was operating right side of face bolter #24. EE had head rolled over to install rib bolt. Employee then rested left hand on the drill while EE retrieved steel from hole sliding the drill mast back pinching the left middle finger. This resulted in a fracture and 5 stitches.
Employee was taking down a stopping and a rock fell out and hit EE on the hand causing injury.
EE was standing on an uneven dirt mound and upon stepping off of it, EE stepped on a pipe, slipped and shifted all EE's weight onto EE's left leg causing EE pain in EE's knee thereafter. EE was limping following the incident, rested EE's leg for a while before deciding to leave.
Employee was the passenger in a rubber tired mantrip traveling on a haul road. The mantrip collided with a motor causing injury to employee's leg.
Miner states around 3am EE hit a pot hole. Reported event went home. Upon returning for next shift EE said still sore. Was seen at clinic given ibuprofen FA treatment. The 26th reported still sore was taken back to clinic suggested to see specialist and to start PT treatment
The employee was measuring silo's and was stepping around a pipe and felt pain in the left knee.
Employee was in the process of changing a tire on a Case Skid Steer loader when EE felt a pop in EE's left bicep followed by pain
An injury to an employee occurred in 7L longwall section involving an electrical shock. Employee was trouble shooting an electrical issue on the 4MVA power box when employee received an electrical shock from an unknown location. Employee received electrical burns on right arm. Employee did not lose consciousness & walked on own accord out of the mine. Investigation ongoing.
Employee was walking to the tool car when EE stepped in an area that was slick due to water and slipped and fell hyper-extending EE's right knee causing injury to knee.
After drilling a hole employee placed a PVC Collar pipe into the hole. While pushing the PVC pipe into the hole with EE's left leg EE felt a cramp causing EE to fall. Employees leg was dislocated and later found to be fractured.
Employee slipped and fell in the night shift parking lot when reporting for night shift. Employee didn't want to be seen by an EMT, just wanted it reported. Later the employee was brought to the hospital for evaluation. Employee was removed from work.
Pulled a muscle in right shoulder while cleaning out dirt from the rod wiper on the drill.
Employee stated that EE was using a sledgehammer to knock a stopping when the injury occurred, Employee was knocking a stopping and the employee was ""choking up"" on the hammer handle while swinging into a block stopping causing EE's finger to be caught between the stopping and the hammer handle causing a laceration below the knuckle on employees left index finger
Working in a squatting position for 30 - 40 minutes wiring a crusher motor. Knee began to ache but finished up the shift. Pain intensified on the way home.
Employee was in the process of changing out a dewater pump at the boot wash station at the bottom of the shaft. They accidentally stepped into the pump hole when working in the area and submersed their leg up to the knee.
Employee was hooking a steel rope up to pull belt off framing, when employee felt pain in lower right back. Employee started missing work on January 2, 2020.
Employee was climbing up step of rock truck and fell back onto ground.
WET, MUDDY, UNEVEN BOTTOM. DRAGGING CURTAIN, FELL GRABBED A RIB BOLT TO CATCH SELF AND SEPARATED BICEP.
A fully trained miner was using a panline bolter when the miner got a finger caught between the drill head and a drill steel. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was attempting to remove a dolly pin from a draw bar before the tie down chain was bearing the weight of the dolly and the dolly coasted, forcing employee's finger between the top of the pin and the mantrip resulting in four stitches to right index finger. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was scraping mud off the haul road with a front end loader when EE hit a rock with the bucket. It stopped EE immediately and the loader twisted sideways resulting in a strain to EE's lower back.
EE was walking from upper office to lower mill when EE slipped on the ice and fell.
Employee was taking slack out of cats on #2 MRS . EE went to remove grease hose from the grease fitting, the grease fitting broke causing grease that was under pressure to penetrate right hand.
The employee was traveling back into the section with their ram car when they hit a pot hole and hit their head on the canopy of the ram car.
During the coarse of the day the employee started feeling pain in their right knee. At one point felt a pop in their right knee. The employee continued to work until 01/15/2020 when EE was taken off work due to medical advice.
Employee was lifting a ""snake head"", at the tailgate, to get it out of the way. EE felt a pain in EE's side as EE moved the snake head.
Employee had pulled roof bolter up in #9 entry on 001-1 section to prepare to start bolting place and was bending bolts to be installed first when hand slipped off roof bolt hitting hand on ground and roof bolter.
Employee was walking on the second floor of the train load out to trace out a strip of heat tape for the load out waterline. The employee tripped over a used motor and pump that was out of service sitting on the floor. The employee landed on elbows causing pain to left elbow and shoulder.
EE was working on wheel unit of shuttle car when tire moved falling onto EE. EE injury was changed to reportable when EE's condition was downgraded during transit. Cause of this incident was lack of concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency was a factor.
While installing a cable bolt in the #2 entry, the cable bolt bent and struck employee on the left forearm.
EE was trying to dislodge oversize, a rock hit EE hand causing a fracture to rt hand middle finger.
Had a bit break off in end of coal drill auger, myself and IE were beating old bit with hammer. Piece of bit broke off hitting IE in the eye, we both were wearing safety glasses but the piece that hit EE busted EE's glasses.

Employee operating skid steer when it suddenly pulled hard to one side causing employee to hit their head on side of window and later experiencing neck pain. Employee did not report incident and went to ER on their own.
Employee was standing on coal pile behind miner hanging the laser when they tripped and fell. EE caught themselves with left arm causing pain in left shoulder. Employee worked till 12/10/2019 when they were taken off work for the shoulder injury.
Employee was guiding a shield hauler in to pick up a re-lay bar on a shield. Employee told the hauler operator to ""tilt down"". After the hauler tilted down, the employee misplaced right foot, and it became mashed under the left fork of the hauler, fracturing right Big toe.
Noticed eye irritation at approx. 11AM 12/5/19. Did not report to supervisor until 12/7/19 10AM.
Raw FK line that transports material was plugged with rock and wet material. Employee went to check line on top of electrical room. Employee then removed cap while line was pressurized. Left ear, Face and upper body impairment.
Employee was in the process of using a pry bar to roll the roll-away box back into place after performing maintenance on the shaker. The pry bar slipped out of the pivot point and EE's right hand contacted the right edge of the roll-away box. Although EE was wearing gloves, the force from the contact partially tore EE's right pinky finger nail and drew blood from the nail bed.
While operator was walking portable conveyor into place, EE inadvertently rolled the track over left foot. Fracturing EE's left foot (toe).
Employee was injured by having a drill hammer mash EE's foot which resulted in a fracture.

Employee was operating a 20 ton motor when employee hit a kink in the rail causing employee to jam right side into the frame of the operator's cab. Employee suffered a fractured rib.
Employee and 2 others were in the process of recovering belt structure. While breaking down the belt structure, they had to remove 6"" aluminum water line that was hanging from the mine roof with belt chains. As employee and the other 2 employees was lifting the water line up to remove the chain from 3rd joint they were letting it down it fell and struck employee on the left ankle.
The employee was throwing the K4 track switch along 5 East Haulage, when they felt pain in right shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
Employee had finished removing scaffold and was climbing down from the building structure. Employee lost balance and fell approximately 8 feet to grade. The employee suffered a fracture of the left tibia and fibula.
Employee was walking up steps to drum feed 3 conveyor when right foot slipped off step and foot landed on previous step (no fall). Felt pain behind right knee area and swelling. Area was well lit and stairs were clear & dry. Employee was diagnosed with a blood clot in the back of the right knee.
Craftsman was attempting to orient a hydraulic cylinder for hyd shovel. The cylinder was on a flatbed truck in a shipping jig. craftsman rigged the cylinder while another operated a crane to position. The cylinder came off of the shipping jig and rolled approximately 1 foot contacting the craftsman's lower legs and feet. Resulting fractures to right and left ankles
Employee had finished drilling hole for a rope bolt. As EE reached to get rope bolt it became entangled. EE jerked on the bolt to get it loose resulting in pain in EE's lower back. EE was taken off work on 12/9/2019.
Employee was pulling torque arm out of belt take-up motors, the torque arm came out all the way unexpectedly and fell on top of left foot. Causing a minor abrasion and contusion of the left foot.
Employee was carrying monorail down the belt when they slipped/fell on some mud fracturing left ankle.
Blisters on feet due to friction inside of boots. Blisters infected - Cellulitis.
Miner slipped on ice and fell walking to office at end of workday.
EMPLOYEE WAS GETTING OFF OF A MAN TRIP OUTSIDE WHEN EE TWISTED EE'S LOWER BACK
The employee was carrying roof bolts from the back of the roof bolter to the front of the roof bolter. The employee tripped and fell to the mine floor, fracturing their right thumb.
Employee was pulling a rolled up jump cable into a mucker bucket. Their hands slipped off the cable and they stepped on a rock and felt some back pain. EE was seen by EB medics and referred to the ER for evaluation. EE was issued restrictions and taken off of work.
EMPLOYEE STATED THAT THEY WERE SHOVELING OFF EXCESS BUILD UP OF MATERIAL ON TOP OF WEIGH BIN, WHEN EMPLOYEE SLIPPED IN MATERIAL CAUSING EE TO FALL AND LAND WRONG ON RIGHT ANKLE. EMPLOYEE WAS TAKEN TO ER 12-2. EMPLOYEE WAS SEEN BY A SPECIALIST ON 12-4-2019. GIVEN TREATMENT PLAN OF CAM WALKER. EMPLOYEE BEGAN MISSING WORK 12-5-2019 FOR LOST TIME. EMPLOYEE HAS FOLLOW UP 12-18-2019
Ee was doing regular rounds, taking reading in pond #1. EE slipped and fell, landing wrong on left foot. No fracture, but diagnosed with contusion and sprain to left foot. Given boot and crutches and site is unable to accommodate restrictions.
While exiting a man van, the employee slipped on the step and fell to the ground. EE initially claimed to be okay, but after completing the pre-shift inspection and getting to the cab of the truck, EE felt dizzy.
Employee stated that EE was using the canopy of the longwall shield to lift a relay bar, during the procedure the tip of the shield came in contact with a built set of cribs the tip of the canopy slipped off the cribs and came downward in a spring-type motion which caused the employee to be struck by the bottom side of the shield canopy and the top of the pan line.
Stepped up on curb and slipped on snow covered ice, breaking left wrist.
Employee was swinging sledge hammer and strained right shoulder. Taken to doctor for examination and EE was placed on light duty.
Employee reports EE was driving 10 mph across railroad tracks and didn't realize it was dug out and as the truck went over the tracks, the truck suddenly stopped, seatbelt didn't lock up, causing EE's head to hit the rear view mirror and EE's back to lock up.
The employee went to remove an aluminum basket out of the back of a truck and felt pain in left arm.
Cleaning snow off of # 9 conveyor belt slip on a patch of ice on grated cat walk started to fall and caught the hand rail under the left arm pit and damaged left shoulder.
Injured was connecting split steel and drill pot caused steel to bend and strike injured on the index finger. X-rays revealed finger was broken and injured was placed on restrictions.
Employee injured back while dismounting a 772G road grader.
While climbing ladder on Rubber Tire Dozer, employee lost footing with left foot, then EE's left hand slipped off leaving two points of contact. EE swung to right side with right arm being extended and felt the elbow pop causing pain.
Employee was assisting a mechanic reattach a dust collector hose for the bolter when employee noticed pain in shoulder and stopped the task. Employee went to a physician who prescribed anti-inflammatory and physical therapy for a strained shoulder.
Employee drove haul truck through pot hole, jarring back. Employee continued to operate truck until later in the afternoon when pain began to bother them. Employee was taken to the clinic, and was given restrictions. Delay in accommodating, causing 2 lost work days.
Employee was setting up for a job cleaning out the caustic soda tank when employee walked through what employee thought was water. Upon further investigation, it was the caustic soda that was being drained from the tank. Employee sustained chemical burns to the top of both feet.
EE was coming from the tower to the core building. EE walked up the stairs by the elevator and when EE stepped off the landing on to the road EE felt left knee give out. This was not a reportable until 2/28/2020 when EE missed time after surgery.
While taking 40# bag of coal off stacking table, employee turned to right and attempted to place 40# bag of coal on pallet and felt pain in right shoulder area. Employee was taken to Urgent Care and diagnosed with strain in Right Shoulder.
While an employee was conducting a walk around inspection of a 693 Overburden Truck, the employee slipped on packed snow injuring ankle.
On 1/8/2020 I was notified that an employee was on the way to the hospital for an infection in their knee. After talking to the employee they said they were using the cutting torch when a piece of slag burned their knee. This occurred on 12-10-2019 to the best of their recollection.
Lime sat in silo and plugged over the chute. Before starting to load trucks, employee was attempting to unplug discharge chute from Silo 4 by pushing bar into discharge chute. Material gave way, bar pushed into chute and hand hit part of the chute. Contact with chute broke left index finger.
Employee was climbing fixed ladder on wash plant. Employee did not have 3 points and slipped. Employee fell and landed on EE's right foot. Employee ended up breaking EE's heel.
Employee was removing a chipper from a muck bucket using a sling attached to the hoist. During hoisting, the chipper shifted smashing the employee's hand between the chipper and muck bucket. This resulted in a Laceration and crushing of the employees index and pinky finger.
Two employees did not follow the identified safety controls on two forms they filled out (Fatal Risk Assessment form & Job Hazard Analysis form). One employee placed self in pinch point between swing radius of backhoe bucket and a steel power pole. Operator of backhoe did not stop operating backhoe when operator observed the other employee within the swing radius of the backhoe.
EMPLOYEE WAS WORKING IN DEEP MUD WHILE HANGING GUARDING AT DRIVE. WHEN EMPLOYEE WENT TO GET UP FOOT WAS STUCK CAUSING KNEE TO TWIST RESULTING IN A SMALL MEDIAL MENISCUS TEAR IN RIGHT KNEE. EMPLOYEE WORKED REGULAR DUTIES UNTIL SURGERY WAS PERFORMED 2/7/2020.
Employee was removing last bolt holding hub assembly on right rear of loader. Blocks holding oscillating axle shifted causing axle to kick upward crushing pointer and middle finger on right hand between 3/4"" air impact gun and frame of loader.
EE was operating forklift to move hydraulic hoses. The fork lift arm was extended up with the hoses when the back tires of the lift came up. The EE quickly tried to lower the lift and then the lift started to tip over. The EE end up falling out of the lift and landing on the ground. The EE went to the doctor and was fine but went back two days later because EE felt sore.
Operator dismounting dragline. Guardrail safety chain across ladder access had been unlatched and hanging down in close proximity to access ladder. While dismounting, operator's finger became snagged on the latch on safety chain, lacerating the finger.
EE was removing a 1.5"" aluminum fitting from a steel pipe collar. The fitting broke and separated from the threads inside the collar. The momentum cause the EE to strike an elbow on a 4"" gate valve located behind EE. The injury was treated as a First Aid. EE was evaluated and had X-Rays on 12/17/19. The X-Rays revealed a chipped bone.
EE was operating haul truck and hit a rut/hole in road, jarring them. EE was seen at Golden Health and returned to full duty. On 12/13 EE asked to be seen again and was released to restricted duty.
Employee was coming down the ladder when a leg gave out and when they caught themselves, they strained their back.
Employee was climbing back onto EE's haul truck when EE slipped from the first step back to the ground. Its now known that this caused a small tear in EE's right achilles tendon. The diagnosis and treatment plan occurred on 1/7/2020.
While operating haul truck from N3 to BD4000 dump, operator reported hitting bumps in the road and also that the bed of haul truck slammed down during 2 loads. This caused them to jar and experience lower back pain.
Employee was using a bolt as a punch to drive out a pin and hit hand with hammer
EE was pushing bolts when EE began to feel soreness in right shoulder.
EE was installing fike canisters weighting 133-148lbs on 12-30-19. EE developed pain in back over the next 2 days reported injury on Jan 2, did not want medical evaluation until Jan 7. Dr sent EE back to work with regular duty due to pain subsiding. EE reported still painful on 1-24-2020 taken to Dr. and given work restrictions and scheduled for chiropractor visit.
A fork lift with a barrel dumper attachment with a band and clasp around the barrel was being used to move barrels of balls. One employee reached down to unclasp the barrel before the forklift had come to a complete stop. The forklift rolled forward and the employee caught EE's finger between the barrel dumper on the forklift and another barrel on the ground.
Injured ee reported they were struck in the L wrist by a door when entering maintenance shop, another employee was exiting maintenance shop at the same time. Injured ee c/o of pain in L shoulder
Employee was walking from pickup truck to a piece of mobile equipment. There was ice underneath the snow. The employee slipped and landed on right side bruising rib area.
Two employees were changing cutting edges on the 988 surface loader in the east bay of the Pete Bajo surface shop. The new cutting edge was resting on top of the old cutting edge on the floor, supported by a bolt on one side. The cutting edge slid, crushing the ee's left hand. Diagnosed with a fracture to the left tip of the thumb, and laceration. Put on restricted duty.
Employee was climbing stairs when employee missed a step and struck knee on leading edge of step resulting in laceration requiring stitches.
Miner was crimping water line with banding tool. After completeing the job, EE then pulled the excess band out of the banding tool and the sharp edge cut the thumb and finger on left hand.
IE was moving a pallet of materials with pallet jack. IE was pushing loaded pallet jack with materials into place and strained left lower leg.
An equipment operator experienced a non-displaced fracture to a bone in the right shoulder when pulling on the cab door of the electric shovel.
A mechanic leaned over to pick up some tools and felt a twinge in EE's back. The employee was seen at an occupational clinic.
Ee was driving haul truck, it started to slide down a ramp, turned 180 degrees, and hit a dry spot of ground, coming to a sudden stop. Ee's left arm was jerked by the steering wheel. Seen at the clinic and put on modified duty, no use of the injured limb.
Pinched finger between pusher plate and head of roof bolt, resulting in a fracture.
While ascending spillway stairs employee slipped and fell causing EE to twist EE's ankle. Sprain was treated conservatively. Employee's personal Physician ordered X-ray due to discomfort. Company Physician had X-ray repeated which revealed a fracture. Employee was placed on restricted duty on 2/11/20.
On 12/26/19 warehouse-person picked up a box and was putting it on a cart and felt a pain in back. This was reported to supervisor 12/30/19 because pain continued. Upon reporting employee was taken to local clinic. Diagnosis was a back strain & restricted for 2 weeks to lifting <10 lbs. Employee was placed on restricted duty.
Employee was removing a taper lock bushing from a drive sprocket. Employee was using a pry bar to remove the taper lock, when the bar slipped, striking the tip of EE's right ring finger between the bar and an I-beam. Employee suffered a fracture to finger tip. Employee taken to medical professional, finger was splinted & employee placed on restricted (one-handed duty)
Employee said was climbing onto the 824 J John Deere Front-end articulated loader. EE's foot slipped off from the first step approximately 24 inches. EE was unable to hold onto the grab irons, falling backward onto back. EE also said had muddy boots.
Operator of HT531 was having difficulty with the bed lever and had to press it with force causing their back to hurt.
Opening pickup tailgate, thumb got caught in the handle as the tailgate swung down. Employee suffered a left thumb sprain.
Miner was descending down the stairs of 992 front end loader when they slipped on steps slid down 2 steps but remained upright never falling. The steps were wet from the rain and the miner strained their left lower back.
EE stepped over a hose, the hose blew apart and whipped around, hitting them on the back of the lower right foot, hyperextending right knee. EE reported on the date of injury, but did not seek treatment until 01/03/20. EE was put on modified duty and an MRI was ordered.
While walking along the boom catwalk at the wharf employee tripped and attempted to catch themselves with left hand on the handrail.
EE was removing a liner bolt with an impact drill, when the drill slipped EE grabbed the drill by the socket, the split pin holding the head in place on the socket backed out 1/32"" catching EE's glove & twisting EE's hand, breaking the sesamoid bone in the left thumb. EE went to the doctor on 12-30-19 where it was determined EE had a broken bone.
Employee was walking and tripped on a rock and fell. They reached out their right hand as they fell, causing a broken finger as they landed.
Employee stepped off a bundle of copper and rolled ankle.
Employees were using a sledge hammer to break loose a nut/bolt on a piece of equipment. The hammer bounced back and smashed this employee's finger.
Mechanic was installing an engine oil cooler on a D9-T dozer when the cooler became dislodged falling and striking the mechanic in the face.
Employee slipped and hit right knee on bolt while working in grid box of 830E. The employee spent the majority of the afternoon working in a crouched position in the grid box. When they got up the knee was stiff, hurting and swollen.
Employee was rotating an injector on the shop table to work on other side, as rotating one edge slipped off of table pinching pinky finger. This pinch required stitches.
Employee was lifting a rock that was stuck between the idler frame and the belt. As the rock reached the top of the idler frame, the weight shifted and the rock slid, smashing the employee's left thumb.

Employee reached up to spin 630 mucker to align to fit through work deck. They placed a hand on top of workdeck post, when the crosshead set down on the work deck smashed the finger.
Employee was walking and tripped on frozen ground from tire travel tracks. Employee failed to have proper awareness of surroundings
IE was helping the operator of a skid loader change the bucket. The operator blocked the bucket but the release lever jammed and they could not release the bucket. Another EE had the operator raise the bucket so EE could hit the lever with a wooden block. The lever released the bucket, then falling onto both feet of IE causing a contusion.
Employee cleaning floor hole at the face. Small (size unknown) rock fell from roof or face (location unknown), hit EE in the shoulder. Initial x-rays (12/17) at ER did not show fractures. Employee requested follow up apt. due to reoccurring pain. Apt. 12/31 with specialist found possible fracture, employee given restriction no use of right arm with a CT scan and follow up apt.
EE was wearing rubber boots while carrying a shovel from the 2nd deck of the TK portable crusher. As employee stepped on 3rd to the last step, EE stepped on side of rubber boot causing ankle to twist and EE fell to the 1st level down the three stairs.
Mechanic placed hand in a pinch point between cylinder & pick attachment. Pick was not supported & shifted, pinching mechanic's thumb. Pick was moved to release the mechanic's thumb. The thumb received a displacement fracture & two lacerations, one of which required 5 stitches. The other required 0 stitches. Mechanic was released with restrictions.
Ee was changing out a pump. The ee pinched EE's thumb between the pump and the tank mount. EE was seen at the ER, diagnosed with a fracture to the tip of the thumb, received stitches, and was released to modified duty.
Employee stepped into parking lot near the end of a sidewalk and slipped on a frosty permanent rubber wheel chock. Employee fell on left shoulder. Employee did not seek medical attention until 2/25/2020.
Gear box of shaker screen at crusher system, loosening bolts with torque wrench when middle left hand finger got between wrench and bolt pinching finger causing laceration.
Injured employee (IE) slipped and fell walking down a recently shoveled outdoor forklift ramp. It's believed left hand struck an adjacent guardrail or toe board while falling. Overnight it hurt and swelled so they sought medical attention the next morning. Doctor's x-ray did not show broken bones; IE placed on restricted duty and Rx meds prescribed.
Employee driving a dump truck, was dumping on uneven ground and the truck tip on its side. A loose item in the cab struck EE in the ribs.
A haul truck operator trainee was driving on the dispatch haul road near the 801 Water stand and stated they hit a pothole causing back pain.
Employee reported pain in left elbow after operating 330 with hydraulic hammer for 2 days.
Citation No. 9534984 was issued during a Part 50 Audit with regard to this injury. The employee was trying to remove a piece of mine curtain from a belt transfer point by cutting the curtain with a belt knife. The curtain moved and caused EE to cut left hand. Later in January EE was on restricted duty due to complications.
The EE was working in the shingle shack where they make slate roofing shingles. EE was operating the trimmer saw for cutting the slate shingles. EE was cleaning the cutting table from debris from slate clippings. When EE wiped the surface EE stuck EE's hand in to far resulting in cutting the tip of EE's middle finger.
Employee was using the Shear machine to trim metal when the right index finger was pinched.
The employee was working as a wet plant operator when a sump started overflowing, the employee was on EE's way to turn off a pump when EE had slipped on the wet concrete. When the employee slipped EE had extended EE's left arm to catch self resulting in a fracture to EE's left arm.
EE was lifting a railcar vibrator into place when EE felt a pop on the inside of the elbow. The EE experienced soreness, but did not report the injury to management until December 15th. It was at that time, the employee sought medical treatment and was placed on restricted work duties.
Injured was changing a T-Bar on a roof bolter. While taking the bolts off EE strained shoulder. EE was seen by the Dr. on 12/16/2019 and placed on restricted duty, only, for a muscle strain.
EE was leaving work for the day and as they pushed the building door closed, felt pain in their right shoulder. Tried otc and self-care until 12/18/2019 when was seen by dr and given prescription meds.
While climbing down a ladder on the plant, the employee slipped and fell into the ladder on the right side.
Employee stepped on a piece of rubber on the ground and tripped.
IW was using a shovel to remove material from a lime stacker belt. A few hours after the task was completed, IW stated that their shoulder was hurting. IW was seen at the clinic and diagnosed with a sprained left shoulder joint. IW returned to work the following day on light duty.
Employee was shoveling spillage on walkway when EE felt tightness in EE's back
Citation No. 9570451 was issued during a Part 50 Audit regarding this injury. The employee tripped while hand dusting a face causing EE to twist left knee.
An empty haul truck veered into the side of a loaded haul truck when meeting each other. The trucks were traveling in opposite directions. The left lower corner of the truck cab struck the side of the bed of the oncoming loaded haul truck. The operator of the loaded truck was not injured. The operator of the empty truck complained of a sore knee.
Employee was shoveling beside and under a ramp. EE pushed the shovel under the ramp and mashed EE's right thumb between the side of the ramp and the shovel. This ""strike"" broke the employees right thumb. As a result of the break EE cannot use EE's right thumb for a short period.
Employee was walking down the stairs when EE felt a pain in the right knee. EE immediately shifted the weight off of knee to relieve pressure to get to the bottom of stairs.
Walking on the concrete to get a loader, slick from mud and rain, employee slipped but did not fall. Strained lumbar region.
As an employee was walking down the stairs on the haul truck to clean the headlights, they stepped in a mud puddle and slipped and fell. The employee slid past the stairs landing between the top and bottom set of stairs and kept sliding down the stairs all the way to the ground.
Employee was attempting to remove a pre-strip using a tool designed for this and felt a pop in their right shoulder.
Employee was installing v-belts on the head pulley at a feed hopper. Right hand ring finger tip was fractured when the finger tip was caught between the v-belt and pulley. Employee was provided immediate medical attention, and returned back to work next scheduled shift.
The employee was assigned to replace a hydraulic cylinder. EE felt a pop in the left wrist while using two wrenches to loosen a hose from a fitting on the cylinder. EE was seen by a doctor and was released to restricted duty.
Employee was driving loaded shuttle car back to the feeder from the face when they hit a rock in the road. Employee felt pain in lower lumbar, so was taken into town for evaluation. Employee was seen by doctor, and was released with retractions.
Employee was spotting an employee on a ladder when EE stepped back to look up and slipped. EE struck EE's lip and back while EE fell.
Employee did not report the injury in a timely manner. EE has been treating with EE's personal doctor who is requesting surgery. We accepted the claim today. Employee fell from track hoe on to EE's left shoulder.
While attempting to turn off a 2"" overhead valve, employee stepped up on a pipe and felt a pop in EE's left knee causing pain
Employee stepped backwards and twisted left knee.
Employee tripped on uneven ground and fell forward. Employee attempted to brace for the fall using right hand. The right hand then made contact with a truck door causing the pinky finger to be jammed into the palm.
Maintenance EE installing lagging on a head pulley. EE lying between belts on left side. EE stated that when they went to crawl out, they noticed pain in the right shoulder between the top of the shoulder and neck. As of Dec 9, 2019, the EE was still experiencing pain, went to dr, EE arm/shoulder was put in a sling, and was given prescription meds.
An employee was descending a staircase in the shipping area when they felt a pop in their knee. Pain was severe so employee was taken for medical attention.
Employee was walking through Morenci concentrator and ducked down for a low clearance area. The employee stood up before they cleared the low clearance area and struck their head right below their hard hat.
The employee sustained a finger injury while replacing feeder bottoms. The chain hoist wasn't secured when positioning the plate and came loose striking the employee's finger. The employee was taken to the hospital for medical treatment resulting in a broken finger/laceration.
Left foot to shin got stuck in mud as employee was assisting in the mobilization of quarry saw track. The track moved but employee could not and track hit the knee.
Employee was un-choking an airslide. When material started to flow suddenly, cement got behind EE's glasses & in R eye. Eyes were flushed, & EE was taken to a medical professional, given a prescription antibiotic. EE worked next scheduled day on restricted duty (clean environment) & was released to full duty at follow-up appointment the next day. (1 day restricted)
On 12/5/19 employee experienced back pain while pulling & lifting dragline power cable onto the tub. Employee was treated conservatively; Rx meds were prescribed and MRI scheduled on 1/24/19. Restrictions were issued on 1/28/20.
EE was taking a new return idler from the back of a mule, just as they lifted idler from back of mule to about waist high, mid back begin to spasm. EE immidiately dropped the idler back down to the bed of mule. The roller is aprroximately 52"" long and 90 pounds. Witness observed as EE lifted the return idler and drop it back down almost immediately.
Citation No. 9534974 was issued during a Part 50 Audit regarding this injury. The employee was loading roof bolts onto bolter when employee felt a pain in lower back.
While applying hand brake during positioning of railroad car, employee felt strain and soreness in lower back area. Employee was taken to local Urgent Care for observation and diagnosed with muscle strain.
Employee was exiting the training room, the door stuck and as employee pushed twisted left knee.
The employee snipped the bands off a pallet located in the bed of a pickup with a wire snips and felt a twinge in their back. At first appointment on 12/5, the provider restricted the employee to desk work until an MRI was completed, but did not indicate it was work related. The neurologist provided a report on 2/5 that said it is work related.
Operator was running palletizer for the bagging operation. Bag started to fall from the feed conveyor and operator reached to catch bag. In the process the operator cut right elbow.
Contract employee was arc gouging wear plate from a dragline bucket and as employee finished the cut, the piece of material dropped down contacting employee's lower left arm resulting in a laceration requiring stitches.
Employee was installing ground engagement parts for the bucket of a 980H loader when a piece of metal punctured EE's lower left leg. The employee was treated at the clinic where EE received stitches.
Employee is a Quarry Laborer at the Sabel Quarry. As they were helping another employee, who was driving at skidsteer at the time, place the stone on the pallet, their right ring finger was pinched between that stone and another stone already on the pallet.
Employee was installing an inspection cover on #2 Plattco valve when EE got clinker dust in EE's left eye. Flushed eye onsite, taken to medical professional, given prescription antibiotics as a precaution, returned to work on restricted duty (clean environment) until follow-up appointment. Returned to regular duty at follow-up. 2 days restricted
Employee was riding in the man van when they hit a pothole, and they felt pain in their neck and shoulder.
EE is a loader operator, as EE was climbing down from loader EE felt a pulling pain in the leg. As EE got off and proceeded to walk, that is when EE's leg gave out.
EE was checking on the mill scale carryover screw conveyor for a choke and put EE's right hand up on the drive guard that was over the v-belt and sheaves. As EE's hand was on the guard the sheave rubbed the guard causing the guard to push back causing what the EE described as a jammed finger to EE's right ring finger.
An electrical Employee received a cut / laceration to left knee while reinstalling a cover on a guard.
The employee was pulling plate steel from cooler wall when the plate broke free and struck an employee in the forehead. 4 Stitches were received.
Ee was opening a bay door and a pulley fell from 15 ft above, striking them on the hard hat, then the right shoulder. Seen at the clinic, diagnosed with a laceration to the back of the head requiring 3 stitches, and a contusion to the right shoulder. Released to full duty.
Employee was inspecting trailer jack for installation. Jack was already extended, employee retracted jack by removing pin, not realizing jack was spring loaded, jack quickly retracted and caught EE's right thumb between foot and jack housing.
Hanging and bolting full line of 6"" HDPE pipe on roof. Pipe resting on pipe roller shifted and fell on miner's left index finger causing miner to receive medical attention and 7 stitches on left index finger.
EE swung sledgehammer to cleanout material B mixer drum. Hit left pinky finger on frame of B mixer drum. EE sustained closed fracture. Surgical repair to nailbed.
Ee was lifting a box of crucibles from one cart to another and strained lower back. Seen at ER and released to modified duty on next scheduled shift.
EE was cutting cardboard with a Stanley knife to cover the cooler inlet into the change room. EE used a straight edge on the concrete floor to cut against. The knife jumped over the straight edge and lacerated EE's left thumb that was holding the straight edge still. EE was issued steri-strips.
Employee finished hanging ventilation tubing and while stepping down off the miner the employee struck right forearm against an already made up rib bolt with rib plate causing a laceration. The employee received 6 sutures. Employee was wearing all PPE.
Employee was dismounting from the motor grader while conducting the pre-shift inspection on the equipment. As the employee came down facing forward, EE's left pinky finger caught a tire tread groove and was jammed, causing a possible dislocation and a fracture of 5th digit.
Employee was helping another employee lift a fan to the back of the pickup truck and their finger got caught in the fan cutting it.
Employee lost balance and fell against the truck with left hand (pinky finger extended) causing a joint dislocation. Employee was placed in a removable splint.
Employee was installing a rib bolt off the continuous miner as EE moved the wrench in to tighten the bolt EE smashed EE's right thumb between the wrench and the plywood rib plate causing a laceration that required four sutures.
Employee was cleaning a glass cup that chipped at the top of the glass. EE received a cut on the thumb that required three stitches.
Employee was bringing 50 pound anfo bags from inside the truck to the tailgate. While bending over, employee twisted and felt pain in lower back. Employee sought treatment with Chiropractor on 12-30-19. No work restrictions given.
The employee was leaving EE's desk when EE struck the corner of an open desk drawer with EE's leg

North Railroad switch in North Branford Quarry was frozen. Heat was used to melt the ice on the switch. It freed up but not all the way. Employee used pry bar to see where switch was hanging up. Pry bar slipped and employee fell backward onto right wrist.
Employee was removing plug from haul truck when the wrench slipped and lacerated employee's finger resulting in 4 sutures.
Employee was riding on mantrip going to the section. Was bitten by a spider on the left forearm. Was given antibiotics on 12-27-19 for infection.
Employee was driving pin on a track hoe and a metal chip went through glove and lacerated finger requiring 2 sutures.
Employee was in the process of removing abandoned discharge line that was stuck in the mud, when EE lost balance and fell backwards (same level fall) striking the left side of face on the road causing dirt, sand to enter left eye. Employee taken to local clinic where eye was flushed and RX given for eye ointment for the irritation.
During the installation of a bin pulley on 70-4 an employee (E1) attempted to reposition the bearing. E1's right pinky finger became pinched between the frame and bearing. Contact resulted in a fractured finger tip and a laceration which required two sutures to close.
An iron worker was performing welding tasks when during the work shift received arc flash burns to the eyes. It was determined that the light from the sun was behind the welder and entering EE's welding shield that caused the automatic lens to malfunction.
Miner was removing a guard off a rotor above miner's head. When the last bolt was removed, the guard assembly fell down striking miner in the face resulting in two lacerations above the left eye.
While moving a turbo charger rotor, Employee pinched finger between the rotor shaft and the crate they were setting the rotor in. Resulting in a laceration to the left ring finger.
A fully trained miner was dragging a manifold when EE's finger was caught between the manifold and the stage loader. The injury was not the result of any compliance issues nor were any protective guards involved.
Employee was prepping area for seal building. EE scaled down loose material from ribs, while bending over to clean up scaled down material a piece fell from rib hitting employee in back of head on hard hat. The incident required 5 staples.
Employee was burning brush and slipped and fell on the ice landing on EE's left shoulder.

The contact from the tumbler being placed into position resulted in a laceration to EE's right hand middle finger that required sutures. The employee received a regular duty work release.
Employee was tightening a bolt when the socket being used broke striking the individual causing a laceration.
Employee was helping change GET on 343 shovel. As EE turned to hook tooth to crane on truck, EE twisted left ankle and fell to the ground. Fellow employee helped EE up and called lead man.
Installing pin in spreader bar for dragline rigging, pin started in hole and was not down completely reached to grab pin, then grabbed pin and pin fell into hole completely, which entrapped ring & middle finger, causing them to be smashed. Went to hospital where X-rays showed no breaks, but did result in nine stitches to ring finger.
Employee was sitting on the panline and was having trouble remotely controlling the LW shield beside the shield they were under. Employee slid down to access the solenoids from the problem shield and knocked the shield light off the shield, striking right hand that was resting on the panline. Employee was trained and was wearing appropriate PPE.
While cleaning a buildup of material in a receiving chute, EE smashed left little finger between the hand tool and chute.

Employee was replacing a vibrator on Dryer. After replacing the vibrator employee looked up to see if vibrator was operating, when a small amount of water with very small amount of acid dripped on employee's forehead and running down into the left eye.
Employee injured hand closing a pump house door. Employee immediately returned to work after seeking medical exam. Upon an Orthopedic follow up employee was given a lifting restriction and provided a hand brace by doctor.
Employee was working on the RK3 Pan Conveyor Bucket Elevator when wind blew lime behind safety glasses causing irritation to the left eye.
Employee was assisting in replacing a LW leg jack. Employee reached and handed ""come-a-longs"" to another employee that was in the back of the shield. When employee pulled their hands back out of the shield, their left forearm scraped against leg jack, causing injury.
Employee was using a utility knife to cut a hose, the knife slipped and cut employee's arm. Resulting in stitches.

EE was clearing debris using a hammer and chisel. EE felt something hit leg and felt a small bit of blood on EE's leg. EE reported it to us on 12/16/19, EE was taken to the Dr. who ordered x-rays. The x-ray was read on 12/19/19 and it was found that there was a .3cm fragment. They attempted to remove it and did not find it. This caused EE to receive a single stitch on 12/19/19.
Employee was attempting to lift a transmission, and felt a sharp pain in their back and couldn't stand up straight afterwords.
Employee was helping to reinstall hinge pin on shaft collar doors when they placed a foot in the wrong location, allowing the cylinder base to come down on top of their left foot.

Thought simply tweaked it knee but continued to experience some discomfort & requested a medical evaluation on 1/24/20. Prescript meds given. Report filed late due to employee initially reporting injury as First Aid.
EE was cleaning the riser with a pressure washer. When EE tilted EE's head to the left dust fell into EE's right eye.
Employee was at the Viola Shaft working on top of a 650 scoop to be hoisted outside to the surface, when employee attempted to lower self from the scoop, employee slipped, causing a sprain/strain injury to right knee. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency not a factor.

IE WAS CUTTING CONVEYOR BELT AND SLIPPED WITH KNIFE AND CUT LEG.
Employee was obtaining an item from personal vehicle in parking lot during lunch break and slipped, causing pain in right knee. Employee has history of personal injury with right knee and the slip caused recurrence of pain. The AM after the employee slipped, EE went to the doctor and received a non-steroidal injection to reduce any inflammation. EE returned to work that shift.
employee was removing the head nut from a cylinder as the nut came loose, it dropped on the workbench pinching the employees distal phalanx of the left middle finger between the bench and the head nut resulting in a minor fracture of the finger tip.
On December 10, 2019, employee slipped and struck EE's shin on a toe board resulting in an abrasion. EE received first aid treatment by a physician and released to return to work with no restrictions. On January 6, 2020, EE went back to doctor for a follow up visit where EE received medical treatment for an infection and placed on restricted duty.
Employee slipped in mud and struck the corner of their thumb on the sharp corner of a piece of sheeting.
Employee was cutting belt material when utility knife slipped and contacted their right knee causing a 1/2"" puncture wound. Employee received two sutures.
EE was working on the primary main hopper grizzly. EE believes metal dust fell into eye. EE rubbed the eye. EE was taken to the Optometrist. Doctor removed a metal flake from right eye. Antibiotic drops were prescribed. Doctor gave full release for return to work.
Employee was closing battery lid on fork lift and dropped lid on EE's hand causing a laceration.
EE was inside a kiln pre-heater removing rock, using a chute. While throwing rock into the chute, the employee's hand hit the roof of the pre-heater. The EE's 2nd digit of the right hand (index finger) was lacerated. EE was wearing leather gloves. EE was taken to the local emergency room where EE received five (sutures) and returned to full duty.
Employee was working on a 4"" compressed airline. After draining the pressure of the pipe they began to remove the end cap and was struck by material stored in the air line. Causing material to get into EE's eyes.
Cut finger while connecting to a trailer.
Employee picked up a hydraulic line to clean under it and it broke, striking them in the face. Employee received 5 stitches.
Employee was cleaning up trash. EE picked up a 50 gallon empty glue drum, EE accidently hit the valve on the end of the drum and releasing a small amount of pressure spraying glue on EE's face and running down into EE's eyes. Employee went for medical treatment that evening.
EE was greasing right side hoist cylinder on rock truck. EE set grease gun down to relieve pressure. When EE picked the nozzle back up EE pressed the trigger and the pressure caused the whip hose to fly up striking EE in safety glasses and the grease fitting to strike EE above the left eye causing a contusion. EE received prescription eye drops as a result of the incident.
Employee was hosing along belt line and a rock came off the belt causing injury to EE's head.
Employee was cleaning around the plant in a skid steer working around the surge pile tunnel at the quarry, the weather was wet and the employee was dumping a bucket load of wet sand on to a spoil pile near the tunnel. As the employee dumped the load of material it splashed back and the material splashed on the employee's face running down behind their safety glasses.
EE was operating a forklift and ran over a piece of wood, felt a pain in the lower back, received medical treatment.
Employee was stung by an insect and had an allergic reaction. Saw a doctor and received medication to reduce swelling. No impairment from EE's daily life.
REPORTABLE ONLY - Employee was helping splice belt when EE's left hand was hit by another employee with a hammer causing fracture and laceration to EE's left middle finger requiring sutures.
5500 #3 Station, moving 12""x12""x5' wood shaft dividers, caught right thumb between two dividers.
The employee was using a jackhammer to attempt to free another jackhammer that was stuck in a pile of material. EE's right ring finger was pinched between the two jackhammers. EE fractured the tip of the finger when it was smashed between the them.
Rib blew out and struck miner in the head causing small laceration.
Employee was using a come along to lift belt when the come along handle broke causing the employee to fall forward striking their left cheek off of the broken handle. Employee received a laceration from this that needed 5 stitches.
EE was prying on planetary with a slate bar when the planetary came out. The weight from planetary shifted to the slate bar smashing EE's hand to the ground causing a laceration to the left pinky finger. EE received four sutures to close the laceration.
While stocking UB01 with plates a metal sliver struck through glove on right hand embedding a sliver between index finger and thumb.
EE was climbing a ladder to clean the burner pipe. As EE ascended dust fell from above getting in to EE's left eye.
Injured was trying to remove a drill steel insert out of the drill steel and when the insert broke free it struck them on the cheek causing a laceration that required stitches.
Employee was exiting the porta-potty and rolled EE's ankle
Employee and another employee were closing a garage door and the cable broke causing one of the rollers to come out hitting the employee in the head requiring 2 staples.
Employee was working behind the chopper stacking rock on pallets. While stacking rock on pallets a stone slipped out of their hands. While trying to catch the stone after it slipped out of their hands they pinched finger between the stone that slipped out of their hands and a stone that was on the pallet.
Employee was shoveling walking area on side of barge. Employee stood up and cut lower back on protruding piece of damaged steel.
While removing the tail section door from belt elevator 6, the employee working the manual chain hoist placed EE's hand approx. 1 ft below the chain on the support structure of the elevator. The door shifted causing temporary slack in the chain which caused the chain to shift down and ultimately pinching the employees left thumb between the chain hoist and the elevator structure.
Employee was moving a man door and got a wrist caught between the door and mine roof resulting in a laceration to the wrist.
Employee was closing the door to the panning station when EE's Left index finger was caught between the door latch and the door frame causing a laceration that required #6 sutures to close the wound.
A drill mechanic was cutting a zip tie on hydraulic hoses and accidentally cut their hand with a box cutter razor.
Employee did not report until 12-6-2019. Employee was welding in a closed area on 12-5-2019 with 2 other welders. Back flash while welding, reflecting off walls caused irritation to eyes. Employee did not realize any eye irritation until the next day, then went to Doctor. Eye drops were given to employee. No shift missed. Employee had all required PPE.
Employee was using a blade to cut and remove electrical tape from a cord. As EE removed glove to peel back the tape EE made one final cut on the tape and accidently cut thumb.
Worker was cutting a piece of carpet when the utility knife slipped, resulting in a finger laceration. Required 4 sutures to left index finger to close the wound.
EE was working in the auto shop, underneath a truck, when they felt something fall into Right Eye. EE rinsed the eye, but still felt symptoms. EE was seen at clinic, and Dr prescribed antibiotic for preventative measures and released to full duty.
Two employees were checking welds on a lip shroud. Both employees were leaning on the dipper lip, with their heads turned downward to inspect the welds, when the crane began to lift upwards. They both then stood back from the lip when the clevis broke from the tension, shooting parts upwards between them. The supervisor was struck in the left arm by part of the clevis.
Employee was changing rubber punch plates. The punch plate punctured through the work boot leaving a one inch or less puncture wound that required 6 sutures.
Employee stated that the wet down hose was pushed into the feeder and became wrapped around the pit breaker. The power was locked and tagged out and employee was manually rotating the pit breaker unwinding the hose when right hand got caught between the frame and a pit breaker bit. The employee received sutures in right hand.
Employee was cutting belt with a box cutter and cut EE's leg. EE required 2 stitches.
Slipped and fell on ground while walking and hit head. Received staples in the head and is able to return to work full duty.
Employee was cleaning air slides with air lance, with proper P.P.E. and foreign material got into left eye.
Employee had finished working on the filter housing behind the JNA panel, EE pinched EE's finger between the panel and the visible disconnect handle.
Employee was assisting in removing a oxy cylinder from the back of a tractor when their right hand got caught between the tractor and cylinder. Employee suffered a laceration to the right palm requiring 4 stitches as a result of the incident.
IE was changing bits on the continuous miner when a piece of metal struck the employee in the neck. The object was removed by the doctor and the wound took stitches to close the laceration.
Employee was checking on a haul truck door that had been reported hard to open and jamming all day. While attempting to open the door, the door handle broke and with the force of opening the door, the broken edge caused a 1/2 in. laceration to the employee's right pinky finger. As a result of the incident, employee received 5 stitches.
Miner had moved the stacker conveyor on the auger miner. Miner got out of the cab and the door came shut and hit the miners middle finger.
EE stumbled on flat ground and landed on the buttocks and felt pain in the left knee. EE reports EE did twist the knee during the fall. Went to the doctor on 12/06/2019, Doctor prescribed a mobilizing knee brace for a hyper-extended knee. No work restrictions.
Wile working in limited access area where hard hat could not be worn, employee stood from crouched position after inspecting parking brake area and hit head on bed lift cylinder causing laceration at left eyebrow.
Employee was working on a conveyor when a roller was stuck. EE pulled on the roller and it pinched a hand between the roller and the frame of the conveyor, pinching the hand. This caused a laceration that required 5 stitches.
Employee was asked to move water pumps. EE drove EE's service truck to the first pump. As EE reached down to pull the ring on the cam lock, the ring pulled opened and sliced the employees left middle finger. Requiring stitches to close the laceration.
While loading belt structure, employee flipped the last piece of structure to stack on another section of structure. During this employee smashed the distal portion of their left middle finger between the handles on the frames. Injury required surgery and employee did not miss any work.
Pushing a roller under conveyor, pinched right index finger between roller and steel causing a laceration.
Occupation Illness EE went to clinic 12/23/19, diagnosis ""Allergic Contact Dermatitis"" due to contact with Quicklime on both hands and left wrist. Was prescribed medication. 12/26/19 EHS Manager took EE to same clinic for workers-comp medical evaluation, physician confirmed ""Allergic Contact Dermatitis to Lime"". EE released to work 12/26/19 without restrictions.
Employee stated they were feeling pain in both hands from maintenance activities. Upon evaluation, it was determined that the miner suffered carpal tunnel in both hands. Employee remained full duty until first surgery on 2/20/2020 and second surgery is on 3/5/2020.
Employee experiences pain in EE's elbows while performing EE's job as a roof bolter. This was first reported on 12/17/2019 employee continued to work until 04/06/2022 when EE was taken off work for medical treatment.
Long-term exposure to dust.
The employee was diagnosed with pneumoconiosis.
Retired employee was awarded 10% permanent partial disability for Occupational Pneumoconiosis.
Retired employee was granted 10% permanent partial disability for Occupational Pneumoconiosis.
Retired employee was awarded 5.5% permanent partial disability for hearing loss.
A voluntary health based audiometric exam provided by the company to a miner who has only worked in a job identified to be below the required action level of 85 dBa and is therefore is not enrolled in the hearing conservation program was reviewed by the MSHA Inspector who told the company to report the voluntary exam results showing a shift as a reportable hearing loss.
Employee began to feel pain in both wrists while performing normal duties. Employee was diagnosed with carpal tunnel syndrome and was scheduled for surgery. The carpal tunnel was due to repetitive motion and it was determined that it was work related. Employee became a lost time on 12/9/19
Employee had a recordable threshold shift results during hearing test.
On 12-3-19 an employee received 15% award in occupational pneumoconiosis. The employee is currently retired and does not work at the operation.
Received notification on 12/3/19 that this current employee has been granted an award of 10% for Occupational Pneumoconiosis. This incident was caused by the employee's failure to wear appropriate respiratory protection during mining career. Employee did not violate any rules, regulations, or policies.
Employee has been diagnosed with occupational pneumoconiosis.
I received a call from an EE telling me to call for an ambulance for IE. IE had been digging with an EC480 EL excavator. An EE told me that IE's excavator had stop moving, so the EE's went to check on IE. Two other EE's found IE unresponsive. The ambulance came and pronounced IE expired.
EE was in the employee parking lot after finishing EE's shift and 2 co-workers noticed that EE was acting confused and suddenly fell to the ground. 1 worker took EE's shirt off and put it under EE's head, they noticed EE was going into some sort of seizure so the other worker called 911, the paramedics arrived in a few minutes.
At approx 2:39 PM under cloudy cool conditions, EE had just completed repairs to vibratory screen (S-9) and had descended the catwalk to the ground. EE was preparing to enter the passenger side of the repair truck cab when EE succumbed to a seizure. Paramedics were called immediately and removed EE to seek medical attention.
Employee was crossing the #1 overland belt at the tail crossover. When EE stepped onto the ground EE's right ankle rolled causing a sprain. The employee was scheduled for physical therapy for this injury on 2/19/2020.
Operator had just adjusted packer. Needed to adjust again, instead of turning off machine and completing the adjustment, operator tried to adjust machine with machine still on. Operator should have followed procedure. When adjusting machine from front side, operator's head hit switch that made cylinder move and caught operator's arm. Only bruise. X-Rays negative.
On 12/04/2019 EE of pickup truck was traveling up a haul ramp & for unknown reasons ran pickup truck into toe of highwall. Due to large material at toe it caused extensive damage to pickup truck. On 12/19/2019 EE was prescribed Physical Therapy for stiffness & sore neck and back.
