Accident type, without injuries

A deformation was found in the Crabapple hoist rope during regular examination. The slight deformation was discovered 15ft from the hoist car. The hoist was taken out of service for the rope to be cut and re-terminated.

An ignition occurred in the E32 Continuous Miner section, located in the #2 Entry at Survey Spad# 101+34. During the investigation MSHA cited missing/worn bits on the continuous miner head that may have contributed to the ignition.

There was an entrapment of two miners on Miracle Run Elevator at approximately 6:03 AM. The elevator had a rope grip fault, which caused the elevator to stop two feet from the surface. The miners were off the elevator at 7:05 AM. The fault was cleared by an elevator service company and inspected.

The East North Vent Shaft Rope was damaged when it was pinched between the VSM bucket and the shaft form while attempting to remove an oversized rock from the sinking bucket. The rope was thoroughly examined at 5:20 am and the hoist was taken out of service pending repair to the rope.

While moving the Cat bulldozer off of the spoil to have maintenance work preformed, a fire was discovered under the operators cab floor board. The operator immediately shut down the equipment and fire extinguishers were used to try and extinguish the fire. fire department was called to finish putting out the fire.

A malfunction of a slope car part occurred. NO persons were injured as a result of this incident. Production had not yet started and all employees were recalled to the surface. The primary and alternate escapeways were still available. The car was repaired at 09:45am.

An unplanned roof fall was discovered in the 1WTG Section #1 Entry, 5-6 block. The fall was approximately 57' L x 16' W x 10' H. No one was injured during the fall.

No. 3 Hoist - limit switch was out of adjustment on south brake track; was readjusted; during night muck hoisting limit switch was again out of adjustment due to expansion of brake track, computer shut hoist down.

An unplanned roof fall was discovered in the Main North 6 entry at 190 block. The fall was approximately 16' wide, 80' long and 12' high. The fall was most likely caused due to gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

A fire was discovered at the North Pratt Bathhouse area in a location of old offices that were locked off to the miners.

An unplanned roof fall was discovered in the 6 North Parallel #1 entry at 9 block. The fall was approximately 10' wide, 35' long and 6' high. The fall was most likely caused due to gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

On 1/20 during the workplace exam, it was discovered the Gazebo Lake located above the mine had emptied through a geological formation its entire contents into the mine. Date and time of the occurrence is unknown, no weekend work. No injuries from this accident.

Was notified by the face boss on unit #2 1st Panel West of a roof fall located in #2 entry, XC-29 (21+20). This was inby the feeder on the working section.

At approximately 11:30 p.m. an elevated methane level was detected at one of the monitoring points in the justice mains panel. All employees in the affected area were evacuated. There were no injuries. Both agencies were notified.

An unintentional roof fall was found at approximately 10:00PM in the face of the #4 entry. Approximately 20'x 20'.

The #7 Split Shaft was taken down to change the rope. This was a planned outage and called in before it was taken down. It was also called back in to the hotline when it was placed back into service.

A Roof Fall measuring approximately 15 feet in length, 12 feet in width, and 5 feet thick was discovered during a weekly examination on the #2 Northeast Mains at crosscut #51 in the #6 entry. The roof fall area has been dangered off and timbered. No equipment was involved and no ventilation was disrupted.

At approximately 10:58 a.m. elevated methane levels were encountered while traveling in the return in the justice mains. All employees in the affected area were evacuated and there were no injuries. Both agencies were notified.

An unplanned inundation occurred while mining in the 4F1 panel at approximately 2:40 pm on January 13, 2020. The inundation occurred in the face of #2 entry inby cross cut #4 when an abandoned well was intercepted while mining.

An unintentional roof fall occurred in the #6 entry of the active retreating 002 MMU. A high angle slip fell in the entry above the anchorage zone. There were no injuries.

Power bumps with heavy snow storm, No. 3 Hoist was pulling lead off 4900 pocket when it kicked out, would not reset. Contacted Spencer Engineering on computer faults. Electricians up replaced two electrical components.

The employee stated a 60 ton Komatsu haul truck, had a part failure that resulted in a haul truck fire that burned more than 10 minutes.

THE ROCK FALL OCCURRED ON THE 005 SECTION AROUND 11:45 AM IN THE #5 HEADING CROSS CUT (16) 20 FOOT OUT BY THE FACE FALLING ON THE ROOF BOLTER. NO ONE WAS INJURED. THE FALL MEASURED 18 X 30 X 6 FOOT THICK.

Miner was descending in finish silo personal elevator when car stopped. Elevator service call was placed, technician reset governor overspeed switch and miner was out elevator at 1/9/20 3:00am

Collar door switch at #2 shaft collar faulted out. Due to this fault the hoist operator did not know if the door had closed or not. This made it unsafe to operate.

Smoke was observed coming from the northeast side of the raw coal pile located near the Prairie Eagle East Portal. Actions were taken to treat and remove the affected material.

Due to a power bump caused by a snow storm, a trip coil in the MG set contactor for the #2 hoist burned out. A replacement was found and the hoist was put back into service.

A fall of ground was identified in the 3595 level at the 1430 heading intersection. The fall of ground occurred at the end of the previous shift due to a blasted round in the level and the mine was cleared prior to the blast.

Smoldering coal fire in the coal yard. Heavy equipment operators spent time using FEL, dozer, and excavator to extinguish.

An unintentional roof fall occurred in 3 North Mains 13 to 13 1/2 Wall #0 Entry. There were no injuries as a result of this incident. Conditions contributing to the roof fall are adverse roof conditions and a lateral shift of the strata causing the 8' bolts to fail at there couplers.

A roof fall occurred on the 2nd South travelway in entry #6 at 64 crosscut. The fall was 19' wide x 10' long x 3-4' high. There were no injuries and the fall did not affect ventilation.

#2 shaft hoist had an unplanned outage involving counterweight guides. Shaft came back up and operable at 3:00 pm on 01/05/2019.

At 2:15pm, smoke was observed coming from the area behind the coal tent. Upon further investigation, flames were observed coming from the scrap wood dumpster. The fire department was called at 2:20pm and arrived on site at 2:40pm. Fire was extinguished at 2:50pm.

The East Main North Vent shaft hoist went down due to Electrical issues with the PLC communications. The hoist was repaired and back in service at 2:10 PM.

Lost #1 hoist due to bad upper limit switch. Hoist was returned to full service at 8:50AM.

Contractor truck driver was found lying on the ground on the drivers side of cement haul truck at the east truck access platform outbound lane by Golden Star trucking trainer and trainee at ~ 1:30-1:45 pm.

Struck against a moving object

Operator was operating CAT 615C scraper, hit uneven surface and seat bottomed out. No damage to seat/equipment had been reported on prior daily inspections. Impact from bouncing of equipment resulted in injury to vertebram body (T-12 fracture).

Over-exertion in lifting objects

The injured was working on 9 North Mains Section 1-2 crosscut at 28 block. The employee bent over to move the miner cable out of the way, when they felt a sharp pain in lower back. Did not violate any laws or company polices, and was wearing all appropriate PPE.

Struck against stationary object

Conveyor was blocked with boulders. Employee went through LOTO procedures. Employee removed head pulley guard to release tension in belt with pipe wrench. As employee was moving pipe wrench towards self, EE's hand slipped off wrench and into the pulley as it began to rotate back to original position.

While a row was being bolted the I.E. stated that they rested their right hand in a pinch point and became distracted looking at supplies needed for the next bolt row. It was at this same time that right side operator retracted the boom on the bolter, which caused the boom mounting pin assembly to come to rest against the stop

Employee was in an underground shop and hit self on the head. EE was alone.

Over-exertion in lifting objects

Employee claims to have been attempting to lift up a rock that was too big and overexerted causing a strain/sprain injury to EE's lower back. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Over-exertion in lifting objects

Employee was bending over to retrieve a Big Jack out of the tool tub on the diesel man trip. While lifting the Big Jack out of the tool tub employee felt pain in neck and middle and lower back. Employee was wearing the proper PPE.

EE stepped backwards and rolled ankle. It was level ground and EE did not step onto anything. X-Ray showed no injury and EE returned to work. EE had an MRI on their own and saw a doctor on 3/9/2020. Doctor says EE needs surgery. This incident is now reportable and EE will have surgery in two weeks.

Injured was walking around coal hauler and stepped on uneven surface and sprained left ankle. Injured was placed on restrictions on 2/3/2020 for the ankle.

Over-exertion in pulling or pushing objects

Employee stated as EE was in the process of moving a piece of draw rock that EE had just pulled approx. 4' on the mine floor EE felt pain in EE's lower left abdomen. EE continued to work the remainder of the shift. The rock was 39"" long x 14"" wide by 6"" thick. Employee continued to work until 2/24/20.

Employee claims they fractured a rib and has fluid on a lung, was working on the screen deck.

Struck by falling object

A bore miner was parked in an area under roof support to replace a shear pin. The roof was checked visually and appeared to be in good condition. A 5""x10""x1-3"" thick piece of the roof, in front of the bore miner and inby roof bolts, separated, broke apart and fell. A piece of the material hinged, swung toward the miner, and hit EE.

While surveying locations for conveyor belt support installation, a section of 4 inch water line that was supported by chains fell striking the individual in the head and back.

The employee was removing a broken continuous miner bit by trying to knock it out with a hammer and a punch. Unsuccessful with the hammer and punch, EE used another miner bit in an attempt to remove the broken one. As EE was striking the miner bit with EE's hammer, EE missed the bit and struck EE's left thumb.

Individual was sitting filling out pre-shift books. When EE stood up EE felt a pop in EE's knee. EE went into surgery on 6/3/2020 resulting in a loss time injury.

Belt man was walking to new location to shovel belt. EE was carrying a shovel in EE's right hand. As EE started down a grade EE got closer to the belt to hold on to dust pipe to sturdy self. The floor was wet closer to the belt and EE slipped and rolled EE's ankle. X-ray showed EE fractured EE's Fibula.

While crossing over top of the MBC the operator stepped down and twisted EE's left knee. EE continued to work EE's regular shift and EE's first missed shift was 02/04/2020.

Over-exertion in lifting objects

Employee was lifting a rail onto stinger when EE felt a pull in EE's left groin area. employee received a hernia. surgery is 3-10-20.

While determining which troughing rollers to be replaced, employee caught hand and arm in moving conveyor belt, causing abrasion to left hand and broken left arm. Employee transported to Emergency Room

Employee had exited the safety office, shut the door and stepped out onto dry level ground when EE's right ankle buckled resulting in a fracture. No event occurred to cause the injury. Classification is Degree 10.

When the miner removed a piece of metal from the magnet, the magnet immediately pulled back the metal simultaneously and pricked the thumb of the right hand.

Employee was removing a cutting edge bolt on a loader bucket and it cut a finger. Was wearing mechanics vinyl gloves.

Employee reported twisted knee from stepping out of service truck on frozen ground on 1/29/2020. Did not seek medical attention at that time. Knee did not heal and medical evaluation was performed and employee was held out of service on 10/30/2020 due to medical restrictions on knee.

Lifting a slab of stone, stone slipped, crushing the two middle fingers of the left hand.

Walking in the H-0 Return and tripped on a rock causing the lower back to twist

Over-exertion in lifting objects

Employee was attempting to hang a number of cable bolts on an ""S"" hook on the rib. The ""S"" hook fell off the rib and employee picked the hook with all the cable bolts still on it and attempted to re-hang it on the rib. In doing so employee felt a ""pop"" in their hand followed by a burning sensation. Employee was seen by medic and referred to ER for care.

Over-exertion in lifting objects

An employee was performing the routine task of installing a cross-arm on a power pole. While positioned in a vehicle lift bucket, the employee lifted the cross-arm and felt pain in their left shoulder. Possible rotator cuff injury.

A group of miners were shoveling the HG-13 xc 30 belt. Miner-1 was leap frogging Miner-2 to continue shoveling down the line. When passing Miner-2, Miner-1 attempted to knock debris from a squeaking top roller (center can) using shovel. The shovel contacted the belt rubber and was knocked from hands causing to strike IE in jaw. This resulted in a fracture to IE's lower jaw.

Employee strained back while loading supplies in scoop bucket.

While staging grates in the plant kiln cooler area, employee slipped on a piece of plywood that was placed in the access doorway, causing EE's leg to come down at an awkward angle.

IE stated that EE was examining the belt line. IE slipped and began to fall forward. When EE tried to catch self from falling forward, EE grabbed the belt chain with EE's right hand and hog panel with EE's left hand and then felt pain in both shoulders. The IE had surgery on 5/15/2020 making this reportable.

While installing the next ADDCAR (belt car) in line while mining, the injured employee and another employee was attempting to connect a de-energized 995 volt line connector. During this process, the line connector was inadvertently dropped onto the injured employee's hand which struck against the ADDCAR being installed.

The employee was walking across the mine faces when they encountered a slight incline and slipped a twisted an ankle causing a fracture.

Employee was struck in the ankle by a cleaning bar. The cleaning bar was attached to a minitrac on the onside of a beltline. The IE was on the offside guiding the minitrac operator around a belt crossover ladder. When attempting to get out of the way the IE stumbled and the bar struck IE's ankle.

Struck against stationary object

Employee was using a slate bar to move track rail closer to rib. As employee re-positioned the bar for better leverage, employee hit a finger on the eyelet used to attach the safety rope on the rail car. Employee became a lost time on 2-17-2020. Employee was properly trained and wearing all required PPE.

Walking behind scalehouse when they tripped, stumbled and fell. Dislocated left shoulder and fracture on left middle finger.

The employee was running the continuous miner when the employee was struck by mine roof material.

Struck against stationary object

A fully trained miner was walking off the longwall face when EE hit head on a shield and jammed neck. The injury was not the result of any compliance issues nor were any protective guards involved.

Injured employee was removing belt cradles from a scoop bucket. Five cradles were banded together with plastic bands. Employee began cutting the bands while holding the bundle with left hand. When the bands were cut, the bundle started to fall over. Employee tried to hold them up with left arm but the cradles were too heavy and pulled employee's left arm.

The employee was walking down the #3 heading in the 1WTG Section. The employee's foot got stuck in mud, and when EE tried to step out EE's foot slipped on the dry pavement injuring right leg. No laws or company polices were violated, and was wearing all the appropriate PPE.

Fall from machine

Went to get into the Haul truck, stepped up on the foot board reached for the hand bar, hand bar had frost on it. EE's hand slipped causing EE to fall to the ground.

Fall onto or against objects

Employee was climbing down off of a scoop after placing it on charge and fell into the scoop charger causing EE to strike EE's small left finger. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Fall from machine

Employee fell while cleaning the top of a rail car. Fell into car and fall protection was worn and used. Employee was rescued and seen by the doctor and was returned to work full duty. Later appt on 2/17/2020 it was determined EE would need surgery and was given modified duty and recommended off.

Over-exertion in lifting objects

IE strained/sprained right forearm, possibly up towards/into EE's right shoulder area. This happened as while lifting a 5 gal. bucket with tools (est. 25 lbs.) out of the toolbox storage unit. IE and co-worker drove down and immediately reported the incident and cold pack was introduced to this strained area of the arm.

Fall from ladders

On 1/26/2020 employee was working off of a four foot ladder and fell backwards from the second rung, breaking their fall with arms extended out behind them. On 10/13/2020 the employee had surgery to the Rt. shoulder making this a lost time event.

Struck against a moving object

As the operator pushed down, EE was backing up on uneven ground/boulder and the dozer slammed down which jarred the operator in the cab.

Struck by flying object

Employee was grinding belt structure on shift. After the shift on the drive home, they rubbed their eyes. Employee went home and slept. When they awoke, the eye was sore so they went to the dr. At the dr.'s they removed a piece of metal from the right eye.

Fall from machine

The Employee stated, the EE was exiting an articulation truck. While climbing down the ladder EE fell from the ladder hitting the ground, landing on right hand. Employee was released to return to work in two days, but went for second opinion, and referred for MRI at a later date.

Over-exertion in pulling or pushing objects

Employee was mounting front-end loader and stepped over a mudpuddle to avoid getting feet wet. Strained lower back when employee pulled self onto ladder.

Struck by powered moving object

While switching out cars located in 1 South 4 wall track switch, the injured employee thought the switch was thrown for the strait. The cars unexpectedly went into the switch, made contact with the employee, resulting in four staples to the head and a fractured vertebra.

Over-exertion in lifting objects

While lifting low belt to remove a bottom stand, felt a strain in back, which became a lost time accident 02/20/2020.

Over-exertion in pulling or pushing objects

Employee was climbing down off of loader at the end of the day on 1/24/2020 and felt an elbow pop. Employee informed the supervisor of the incident on 1/27/2020.

Caught in, under or between a moving and a stationary object

EE was installing new set of 48"" idlers on conveyor. Bar in the idler slipped and came down and caught EE's thumb between idler and steel frame of conveyor.

Struck by falling object

While shoveling near the crusher station at the preparation plant, the Miner was struck on the left shoulder by an unknown object or debris. The Miner was seen by a doctor and was diagnosed with a bruise on the shoulder.

Employee reported while exiting a pickup, EE slipped and fell. EE reported pain in the left arm/shoulder immediately following the incident.

Fall onto or against objects

Employee was walking towards hitching post to plug EE's truck in at the end of EE's shift. EE slipped on the ice and spun around striking EE's right hand against front bumper of the truck.

The employee was bending down to clean their roof bolter dust box when they felt pain in their left knee. The employee's knee cap became displaced when they bent down.

Struck by falling object

EE was helping load scoop bucket, piece of rib fell striking EE in left ankle, impact caused EE to fall.

Over-exertion in lifting objects

Operator was engaging a dock level plate when they felt a pulled muscle in their back. Operator left plant to see DR and received a shot for the pain and was told to stay off work for 1 day rest. Operator to return on 1/24/20.

Over-exertion in pulling or pushing objects

Employee was working at 101 wall in 7 West Longwall helping drag hoses through the belt wall to pre-lace for a power move, when employee overexerted and felt pain in back, resulting in a strain/sprain injury. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Fall to the walkway or working surface

Got out of the car, walking over to equipment, there was a thin layer of snow covering a patch of ice EE didn't see the ice and slipped on it and landed with all of EE's weight on EE's knee.

Over-exertion in lifting objects

Sprained shoulder while handling bucket of miner bits. Safety was made aware on 06/17/2020 that injured has began missing work for surgery.

Fall to the walkway or working surface

The employee slipped and fell to the ground when crossing over a drain trench causing a strain to the back, groin and leg.

Struck by flying object

Employee was using a come a long to assist with unclogging crusher. Chain broke striking employee's right arm.

Over-exertion in pulling or pushing objects

Pushing on the boom of a backhoe to line up a pin. Rupture of Achilles tendon occurred.

Caught in, under or between a moving and a stationary object

Employee was greasing bearing with battery powered grease gun. Employee was rotating stone insure proper lubrication when gloved hand got caught in rotation stone and pinkie finger on right hand was pulled thru. Minimal clearance between stone and trunnion causing injury.

Struck by flying object

Employee was operating shearer mining toward the headgate when they were struck by two rocks, one struck left shoulder causing contusion and one struck left hand that was holding the shearer remote causing contusion and laceration resulting in sutures to left ring finger.

Struck against stationary object

Employee lost EE's footing when EE stepped on a rock and strained EE's lower abdomen.

Struck by flying object

While helping to stage dozer tracks by straightening them out, employee attempted to straighten the track out by striking it with a track pin. After striking it three times, a small piece of metal sheared off the track pin and embedded in the employee's right knee

EMployee was tightening down the load on a flat bed truck when the ratchet strap tightener slipped out of the hole causing EE lose balance momentarily resulting in EE straining right foot. Seen site medic, referred to ER then to a specialist where MRI showed a hairline fracture.

Employee alleged to have an on the job injury while EE was in the process of stacking block on a pallet and felt pain in the left groin area.

The employee and two co-workers were removing a 4' x 4' fan housing from a wall opening when the employee felt a sudden pain in left shoulder.

employee was bolting in the #2 heading when a piece of rock fell off of the canopy of the bolt machine hit left arm,requiring 4 stitches

Caught in, under or between a moving and a stationary object

Employee was cleaning on the N side of the shuttle car bin and heard a noise inside the bin area. Employee went to the south side and opened the doors to take a look and inadvertently put a foot on the rail as the wheel was coming by resulting in the right foot being run over causing three toes to sustain fractures.

Employee was watching another employee welding without any eye protection.

Fall down stairs

Employee was working at the ROM when EE slipped/fell down the stairs on the 3rd floor of the Prep Plant landing on the left hip causing a contusion and affecting herniated disc. (Started losing time on 2/29/2020)

A fully trained miner was shoveling under the belt when the shovel came into contact with the belt causing it to come back and hit the miner in the cheek. The injury was not the result of any compliance issues nor were any protective guards involved.

EE stated EE and 2 other employees were sitting an SCSR box on a shelf. The box became stuck and the EE was pulling the box while the other 2 employees were pushing. When the box freed up, it caught EE's right hand between the box and shelf. It caught EE's right middle, ring and pinky fingers. This resulted in a laceration/avulsion to the pinky finger that required 14 stitches.

As they were hitting the stiff-arm pin for removal, the stiff-arm simultaneously dropped pinching the tip of the employee's left thumb. EE was switching the Y09-10 switch when it failed startling them. Employee dove to the ground hitting a knee on rock. Employee was seen by orthopedic doctor on 2/3/20 diagnosed with contusion given a knee brace and taken off work.

Evaluated by onsite EMS then by UPHS Bell ER.

Operator of HT#4 was headed down hill on the Ponderosa Ramp when the truck lost control and hit the berm coming to a stop perpendicular in the roadway. HT#504 was headed downhill on the Ponderosa Ramp behind the #4 truck. As the #504 proceeded downhill, driver indicates applied brakes and began to lose control turning sideways and striking the # 4 truck, sending it downhill.

As EE was walking out to the car in the parking lot, EE slipped landing on right side. A salt/sand mixture and salt is used regularly in winter to limit slippery spots in our parking lot and employee walkways.

Maintenance worker used a torch to cut a bolt off of a conveyor idler. They hit the idler with a hammer and the piece of bolt they had cut off flew up and hit them in the right eye. (safety glasses were slid down on the tip of nose and did not protect from injury.) The result of the incident was a hematoma on the eye and a scratched cornea.

When the ammonia line was charged, during the plant start-up process, the compression fitting holding the flow meter in the pipe failed releasing ammonia into the building. Miner was operating haul truck and while being loaded a large boulder struck the inside of the bed jerking the haul truck and operator cab side to side. As a result, the miner in the haul truck cab reports headache, neck and back pain.

While cleaning up some calcified lime build up, employee attempted to jump on the shovel to better break through the build up, hitting head on a pipe causing EE's neck to jar.

Employee went to grab a 40lb. bag of coal that fell off bagging chute, and felt pain in right shoulder area. Employee was taken to local Urgent Care and diagnosed with strain to right arm and placed on restricted duty.

As employee was cleaning the shop EE rolled a crusher hammer onto a pallet to get it off the floor. The hammer shifted to the side and pinched EE's left ring finger between two hammers causing a fracture. EE also tore away the skin on the bottom of the fingertip.

Employee reported injury on 12/23/2019 and was treated at the clinic and released with no restrictions. Employee returned to the doctor and was given mild restrictions that kept employee working. Injured worker was working on 1/14/2020 when we reported that EE had injured self the previous day while changing crusher shoes. The employee was attempting to pull the roll crusher out onto the service platform. The factory pick point broke causing the the rigging to strike the injured employee in the left fore arm.

EE remembers nothing from the fall or the events that took place after. Said nothing about the fall when talking w/management, but complained of back pains and drove home. No clear diagnosis was given, but employee did receive restrictions.

The date of the incident was 12/2/2019 and was reported by employee on 12/3/2019. EE said right shoulder was hurting, but wasn't sure they did it on the job. An accident report was filled out. After a visit to family Dr. and MRI, an orthopedic Dr. said the injury might be a new injury. EE reported this result to company on 01/21/2020 at 1:56 pm.

Miner was strapping down a conveyor section on a flatbed trailer. As they were descending, foot slipped and they went to reach for a bar that was upright in one of the saddle strap holes. The bar slipped out of the hole and the miner fell to the ground below. The miner sustained impact injuries to right shoulder and hip. Miner was taken to the hospital for evaluation.

Citation No. 9534987 was issued during a Part 50 Audit regarding this injury. The employee strained EE's lower back causing pain and numbness in EE's left leg. EE was trying to straighten a roof bolt as EE installed it at the time.

Employee was helping another employee change a roller that was heavy and in an awkward position while trying put the roller in the frame work. The roller slipped out of their hands. Employee tried to hold the roller from falling and pinched EE's left ring finger tip against the frame work.

EE was moving the genie lift when they hit a bump or rock. The jack leg came sliding across the basket. EE tried to catch themselves, but missed and hit left wrist on the bar.

The offside Pinner Operator was bolting in #2 heading of the new panel. There was a cavity in the roof as they proceeded to drill a hole on the inside bolt a chunk of rock fell onto canopy and slid off striking the miner in the shoulder area. The rock was 4 inches by 10 inches.

The injured EE was helping free a frozen gate latch on the bed of a Mack dump truck. EE inadvertently placed EE's hand between the gate latch and the truck bed and as the air latch closed it pinched EE's right-hand pointer and middle fingers between the bed and gate latch.

On 8 Jan 2020 the employee was working in the Dry Plant area and individual tripped over EE's own feet and when EE fell forward EE placed EE's left hand out and palm of hand impacted a small rock on the ground. The rock made a cut into the hand. Several stitches were given for the laceration.

Employee was placing a 55lb bag of finished product on to a pallet for further packaging. The employee stated EE felt a ""pop"" when EE twisted and placed the material at approximate waist height onto the pallet. The employee was diagnosed with a strained muscle behind EE's left shoulder blade.

Employee was working in an awkward position in the Feed Shute, swinging a sledge hammer to dislodge a liner when EE felt a sharp, shooting pain in the right shoulder. On 01/07/20 while descending the steps from the primary screener, the employee slipped and fell, striking lower back. Employee did not seek medical attention at the time. On 2/3 the employee sought medical attention for aggravation of this injury, and was placed on restricted work duty. Freezing rain that morning was a factor in this incident.

Operator of HT619 was getting loaded at the 16 shovel when they were rough loaded. Operator felt discomfort in lower back but continued operating the haul truck. Employee waited until next shift to be evaluated.

Employee was bolting off the miner. As employee bent down to put a bolt in the wrench some rock fell off the edge of the roof screen and struck employee in the right side of head, neck and shoulder. Employee suffered a laceration to right ear that required three sutures.

While helping Mechanic change out a bushing on dozer, bushing came out, pinching and cutting left index finger. Three stitches and glue was required.

While using torch to remove and replace shaker screen, a piece of debris hit the employee's hard hat and fell behind cutting PPE. Employee was taken for evaluation at medical facility and returned to work with no restrictions.

EE was washing a piece of equipment when the EE sustained a cut on the thumb from a metal burr.

This was a truck driver driving a tandem dump truck. EE had a rock stuck in the tail gate and decided to open the tail gate with hands to get the stuck rock out. Then EE didn't move hand fast enough and the tail gate slammed closed on EE's finger.

Employee was putting up a pin with a board and grabbed the board to straighten it. The tip of EE's finger was pinched between the board and the roof.

Struck by falling object

Employee was beginning to move a green two wheeled recycling container. When they tipped the container back to move it, the left wheel fell off and the container fell on top of the employee. Another employee assisted getting the container upright. This became a recordable when prescription medication was prescribed on 1/14/20.

Over-exertion in lifting objects

The employee works in a position where frequent shoveling is required. EE's shoulder has been sore for a while and aggravated by shoveling. Is experiencing tingling in EE's arm when trying to sleep. Went to Dr. who says scar tissue has built up and prescribed 6 weeks of physical therapy.

Employee was cutting a water discharge line with a utility knife and contacted their left index finger causing a 1/2"" laceration resulting in four sutures.

Working on #6 Scoop. Employee was pulling cable leads through rubber grommet with channel locks when they slipped loose and channel locks struck above right eye. Employee was taken to ER were EE received 3 stitches.

Hanging miner cable and tie wire came loose twisting EE's arm back. Causing EE to have surgery on March 3, 2020 to repair tendon.

Employee was adjusting belts on de-oiler pump under float plant. Employee felt a slight burning in right eye, washed the eye at eye wash station but continued to feel a particle in the eye. Employee was taken to NFEC a piece of metal was removed from the eye. Employee was given a prescription medication.

While being loaded with blasted stone into the haul truck, a stone from the kalkberg bench caused the truck to jolt which then caused the employee to experience a ""pop"" in the neck. The employee stated that EE initially felt pins and needles but that subsided by the time EE got up to EE's phone to inform the supervisor.

Contact with hot objects or substances

While harvesting the #11 cell, one of the anodes had two pieces of copper stuck to it. As the employee was attempting to dislodge the second piece of copper, the anode tipped over knocking the employee to the floor. As a result, the anode contacted the right arm of the employee causing a second degree burn.

While sliding drill bit basket on drill deck, caught hand between basket and handrail.

Struck by flying object

Employee was working on a barge pump releasing pressure from the pump line. The hose blew off, pushing EE into a handrail on the barge pump. This resulted in a fracture to EE's left #10 rib.

Employee was adjusting forks on rough terrain forklift and smashed middle finger on right hand between fork and frame. Required 6 stitches.

Employee was opening a box with employee's utility knife when the knife slipped cutting employee's left thumb. (4) sutures were required to close the wound.

Fall onto or against objects

IE was packing 60"" top belt rollers to supply trailer when IE tripped over a stick of 6"" waterline. IE fell into the supply trailer striking IE's left knee and thigh. IE received a 1"" laceration on IE's inner left thigh, which required 6 sutures to close the laceration.

A fully trained miner was carrying a grinder when the miner accidentally hit the trigger switch causing the grinder to cut miner's hand resulting in sutures. The injury was not the result of any compliance issues nor were any protective guards involved.

Over-exertion in pulling or pushing objects

EE was raking kiln 2 cooler grates when in pulling the rake back, the handle struck a hand rail behind EE causing a left shoulder rotator cuff strain. EE reported the discomfort and was sent to company doctor. EE was diagnosed on 02/06/20 and physical therapy recommended.

Employee was attempting to take water supply tree off 4"" main water line, when it came loose and struck them in the chin

At approximately 11:50am at Young Mine a miner had entered the 7690 heading after mucking activities, checking the area, when an approximately 100 pound rock fell striking EE on the head and arm. The rock fall occurred on the left-hand rib, where the rib meets the face, about 10 feet up the rib line.

EE was performing maintenance on a set of screens. While inside the screen deck EE looked up and a foreign object fell between EE's safety glasses and EE's eye. EE washed it out and reported to supervisor. The next Monday (1/13/2020) it was bothering EE. At this time supervisor took EE to clinic.

Team member stepped in a rut at the facility and twisted an ankle. The team member was taken to the ER and diagnosed with a sprain. At follow up appointment MRI was prescribed and determined a hard boot should be required on March 5, 2020. The team member continues to be able to perform work duties and has not missed any work.

Struck by falling object

Employee was building a Kennedy Panel regulator in the 6th North return. As EE was attempting to place a Kennedy flag on the upper corner of the regulator, the panel slipped down and cut EE's left index finger. The laceration was glued and employee was given a prescription by the doctor.
