A roof fall occured in the old 2nd developement of the 002-0 section. The fall occured in the No. 6 intake entry at survey station no. 4868. The fall measured approx. 30' long x 20' wide x 8' thick and fell above the anchorage zone of the 48"" fully grouted bolts installed in the area. The fall occured approx. 3000' inby the active section.
The traveling cable was damaged on the underneath side of the miralle run elevator by rubbing against an I beam cable was replaced and elevator restricted to normal operations at 4:51 am 6/2/06.
Unintentional roof fall #2 entry, #13 crosscut intersection in #3 south submain, 25' long x 25' wide x 9' high. Date of development 12/14/2005. Bolted with 8' tension rebar bolts. Called in immediately to office in Hillsboro.
Unintentional Roof fall #1 entry & #61 crosscut, 5th east panel. 25' long x 25' wide x 9' high. Date of development 2/2/06. Bolted with 8' tension rebar resin bolts.
A roof fall was discovered in the East Main entries crosscut 67, between entries 6 and 7. The roof fall measured approx. 18 feet wide, 80 feet long and 8-12 feet deep.
An unintentional roof fall occurred in #1 entry 4400' outby face on 3rd northwest mains, 50"" in length, 19"" wide & 6'thick. 200' inby spad # 983 in return air course.
In a face where the bore miner was cutting the top caved on to the bore miner. The operator stayed in the miner until the fall was bolted and the operator could remain under bolted to to get out of the miner.
An unintentional roof fall occurred in xcut between #4 & #5, entry 4300' outby face on 3rd northwest mains. 30"" in length, 19'wide & 5'thick at spad #1087 & 1086 in neutral air course.
Mine foreman found roof fall in 1B return area. Fall is in 3-way intersection, 25' long, 18' wide, 5' - 6' high. Fall will not be cleaned up. Primary roof support was 5' double rock bolt. Area will be timbered off.
A roof fall measuring 19' wide x 120' long x 5' thick was found in the neutral entry 7800' from the working section at spad 38368. The fall occurred due to intermixed coal streaks and rock above 4' resin roof bolts. No injuries occurred as a result of the fall, ventilation was not affected nor the passage of men. Area was developed March 2001, fall was dangered off.
A roof fall was found at 11:30 AM on 5/30/2006, located at survey station 11+20 entry 6 in 6LMS. Fall was outby working section and area had been mined Feb.2005. Fall measured 20x20x10 high and had been supported with 8' full resin pins. No injury resulted but an intake stopping was destroyed. There are no plans to clean up this fall.
Mine foreman found roof fall in return 1B area between +585 & +450 #2 entry. Fall is 35' - 40' long, 18' - 19' wide and 5' - 6' high. Fall will not be cleaned up - Area has been timbered off. Primary roof support 5' double lock.
On 5/30/06, an unintentional roof fall occurred at the #6 crosscut in #7 entry, this was located in the 1st West/4th North, return. The area was initially supported with 10' point anchor roof bolts, the fall measured 21' X 17' X 10' high and the roof fall will be cleaned up.
Roof fall crosscut #3 & #4 entry, 2nd west panel 350' mark. 30' long x 18' wide x 9' high. Date of development 5-25-05. Bolted with 6' & 8' tension rebar bolts.
A roof Fall occured measuring 20' x 20' x 0 - 10' in the intersection of #3 entry. No equipment was involved, no ventilation was disturbed, no injury occured and the fall will not be cleaned up.
Employee was operating 740 cat articulated truck going around small curve, when truck run upon 5 ft. berm. The truck cab started coming back on road when the truck bed crossed berm pulling truck over berm causing truck to overturn on its side bending cab and breaking glass. Operator was wearning seat belt. Operator said he had looked off when accident occured.
Wana service elevator stopped ^ 125' off top landing. I-beams, rail clips and counterbalance guide brackets were repaired.
A roof fall was discovered at 12:40 p.m. The fall was approximately 100' long x 18' wide x 6' wide. The fall will not be cleaned up.
North left section MMU 004, #3entry,19block,4way intersection 25ft long x 17 ft wide x 7 ft high. Suppor: 6ft 3/4 inch resin bolts, screen, pans, drywldg truss bolts,straps, 6-12 ft cable bolts.
A roof fall was found in the main north entries in #1 entry between #12 and #13 x-cut.
A roof fall occurred in the 14 North panel, intersection of crosscut #18. The fall measured approximately 50' x 19' x 5'. The area was supported with 5' fully grouted bolts. An undetected slip was determined to be the cause of the fall.
An unintentional roof fall occurred on the 2nd Main West at 56x6, in the #3 entry. The fall was part of a crosscut and an intersection. The fall was approximately 30' long, 18' wide and approx. 5' high. The fall will be timbered & dangered off.
A roof fall occured in the 14 North Panel, #10 entry, starting in the pillar line between #10 and #11 crosscut and ending in the middle of the intersection of crosscut #12. The fall measured approximately 100'X18'X6'-8'. The area was supported with 6' tension rebar bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.
North left section MMU 004, #5 entry, 20 block,4 way intersection 25 ft. long x 17 ft wide x 7 ft high.Support: 6ft 3/4 inch resin bolts, screen, pans, dywldag truss bolts, straps, 6-12 ft cable bolts. Fall dangered off additional roof support installed around fall.
Xcut north at 1920' on 1A road (roof fall) intersection on inby & outby end of fall had previously been cable bolted roof fall 6' high 17-18' wide 35-40' long area that fell was bolted with 5' double lock bolts.
EE (customer and employee at Littl'fellows trucking in Palmetto) was on his way to the scale house, when he reached for his lighter on the bottom of his truck and drove over a berm. The truck ran into a ditch and rolled over. The driver was not injured.
Fall of roof - B entry 2nd west @ SS-27+20. Approx 24'L x 28'W x 6'H. Grey Shale. 4 way intersection. 6' resin grouted rods.
12CM27 miner was cutting rock in the no. 1 entry on the no. 6 section when a methane bleeder was ignited causing a frictional ignition on the left side of the no. face. The ignition lasted approx. 1 minute and the flame was extinguished with a fire extinguisher and water.
The elevator at our Hart street portal failed to operate when the cable system on the elevator doors broke. This stopped the safety switches from functioning taking the elevator out of service. The door cable system was repaired and the elevator returned to service at around 10:00 A.M.
An unintentional roof fall has occured on the C-Mains #1 entry, between spad stations 13420 and 13406. Height - 10' Width - 18' Length - 30'. The area was supported by 5' fully grouted resin w/ 20"" pizza pans and 10' cable bolts. No equipment damage or personnel injuries.
An area of the mine was timbered due to bad top November of 2005. Some more rock fell during an unknown time. There is still some bolts hanging down where the rock had not fallen completely above anchoring zone. The fall is located in a primary escapeway and did not exclude the passage of men or ventilation and was outby the working section. The travelway was moved up one break.
An unplanned roof fall occurred in stockton mine between 84 & 85 break (Rich Hallow Track). The fall measured 40'long x 20'wide & 4-8' in height. This mine is idle.
Roof fall found on weekly exam by MSHA & mine foreman 1 west mains, E panel, 10 block inby spad 5417 between #4 entry & #5 entry. 50' length, 16' wide, 6' high.
an unplanned roof fall occurred in the #1 entry between spad 02716 and 02728 in the tiller return. This is an idle mine, with no production. No injuries, no equipment involved. Passage of persons & ventilation were not affected. Fall measured 20'l x 20'w x 48"" h. Fall was inspectted by wv inspector.
A roof fall occurred in the 2nd east entry in #10 entry spad 69+20 measuring approx. 100' x 20' x 6'. The area had been bolted with 5' fully grouted roof bolts in early May of 2006. The fall was apparently caused by a hidden slip in the mine roof.
On 5/23/06 an unintentional roof fall occurred on the 3rd south return in #2 entry at #12 cross cut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 18'l, 18'wide x 15' high and the roof fall will not be cleaned up.
A fire boss discovered a roof fall in the #2 entry escapeway. The fall was not above the bolt anchor but did impede passage.
Elevator not working properly. No injuries or accidents. National Elevator was called. They repaired switch. Elevator is now functional.
Electrical power pole caught on fire and could not be extinguished until the power company came out and de-energized the line to this pole. Did not involve any property owned by Hanson, but took more than 30 minutes to put out.
A roof fall was discovered 50' from station 6244. The fall was a continuation of the gob fall on the longwall section. The fall will not be cleaned up. The fall was approximately 30' long x 18' wide x 6' high.
A power outage due to weather disabled hoisting while 5 employees were underground. Before utility company could energize alternate lines, they had to be checked. Approximately 80 minutes. MSHA inspectors on site at time of incident.

roof fall at station et 15+30 in #5& 6 entry in 1st north panel. Fall was 6' high, 20'wide and 60'long. No equipment was involved in the fall, the area was dangered off and post was set around the area.
12CM27 miner was cutting rock cleaning and trimming the right rib in the no. 1 entry on no. 6 section when an ignition occurred that burned approximately 15 seconds. No injury or property damage was incurred. The flame was extinguished with a fire extinguisher.
THE TAILGATE ENTRY ON THE 8 LEFT LONGWALL PANEL BECAME IMPASSABLE DUE TO A ROOF FALL MEASURING 10' W, 6'L, 8'HIGH. ALL EMPLOYEES WERE REMOVED FROM THE FACE AREA AND TRAINED ON SCSR'S AND ESCAPE ROUTES. WHEN MINING RESUMED, 2 PASSES WERE REQUIRED TO MINE PAST THE FALL.
Fall on #2 unit, #3 entry, 2nd open. Water & sandstone were visible after the fall. Length 30', width 35, thickness 7'.
An unintentional roof fall occurred at the Galatia North Portal on the #1 entry, 13th West Longwall at 3977'. The fall was approx. 15'long x 17'wide & approx. 20'thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up.
Coal barn fire. Fire Dept. notified at 5:50pm, arrived at 6:03pm and had fire controlled (extinguished) at 9:50pm. It is beleived that coal ignited due to spontaneous combustion. No injury/illness/accidents resulted from this incident. Minor production loss & material loss. (coal)
Had an unintentional roof fall between break #2 & #3 on the #18 belt line. Fall was 10' high x 20' wide x 45' long.
Rock fell 8' long x 41/2' wide to 5 1/2' in the middle to 0"" to the edges thick at no12 entry next to face about 12' from face.
A shot was put off at aprox 2:00PM Friday that conected 27 South with 27 East going West. This area has been a known problem and the shot was allowed to set over the weekend. Size of fall: 50'x70'x10' It did not affect ventilation of passage however it was above the 8' & 9' bolts. The day of the fall was not a scheduled work day. Pumping only.
THE WEEKLEY ESCAPE CAPSULE WAS INOPERATIVE FOR AN EXCESS OF 30 MINS. DUE TO A PROBLEM WITH THE BRAKE MODULE RECTIFER. THIS CONDITION WAS FOUND BY AN INSPECTION OF SUCH CAPSULE.
PC1800 was digging on a hard place in the pit. The bucket came in contact with an unfired booster and set it off. The bucket was on top of the primer. No damage just a few rocks sprayed in the immediate area. Also a loud bang.
A roof fall occurred on the 3rd Main North Belt between 33 & 34 xcuts (03 Entry 103+90'). Fall was 18'W, 10'L, 6'H. Area was mined in Nov, 1993. Fall occurred due to deterioration of roof around existing roof bolts. Area was supported with 8' long point anchor roof bolts using 2' of glue & an anchor shell. Fall blocked the passage of material on the belt.
At 4:10 pm a may day was called due to a lightening strike grass fire located on the hillside east of the #2 tailing dam, away from any active mining or reclaiming but on mine property. The ""fire out"" was called at 4:43 pm. There were no injuries. MSHA Albuquerque & Dallas offices were notified immediately.
A roof fall occured in the 3 East panel, crosscut #11, starting in the pillar line between #1 and #2 entries extending halfway through the pillar line between #2 and #3 entries. The fall measured approximately 65'X19'X6'-7'. The area was supported with 5' fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.
The back at 53W 15100 D3 fell out with the raise at 48R BS. Because this was initially thought to be a ground fall, MSHA was contacted. Upon further investigation, it was found that the combination of pre-drilling and blasting caused a portion of the back to come in. This was evidenced by bent ground support in the hanging wall. No one was working in the area at the time.
5/18/06 an unintentional roof fall occurred on the #4 Unit, MMU-004, #3 intake entry at #29xc. This roof fall was located in the intersection & was supported with 6ft point anchor roof bolts. The fall measured 25'L x 18' W x 7-8' in thickness. The roof fall will not be cleaned up and all approaches have been cribbed off and flagged off.
Malfunction of the elevator occurred which it was inoperable for 60 mins. Malfunction was electrical wiring to a door switch.
A roof fall occurred in the 11th north panel in No. 5 entry (supply road) at spad # 3 + 70. 50 ft. long X 18 ft. wide X 6 ft. high. Did not fall on any equipment or affect ventilation. Working section moved on 5/11/2006.

A roof fall occurred in the main east main at x-cut # 150 at spad station # 106 + 13 in the # 1 entry. 35 ft. long X 18 ft. wide X 6 ft. high. No equipment involved or ventilation affected.
A roof fall occurred in the main east main at x-cut 155 Spad station 109 + 63 in the # 4 belt entry. 55 ft. long X 29 ft. wide X 6 ft. high. No equipment involved, did not affect ventilation. Working section moved on 5-11-2006.
Discovered roof fall in primary escapeway measuring 50' long x 18' wide x 7' high. Area was timbered and dangered off.
At 11:40 am a roof fall ocurred outby section approximately 1100' (outby section) in # 7 intake entry. Fall is 20' wide, 20' long and 8' thick. No one was in thia area. No injuries or equipment involved. Area will be cribbed off and left. Section will be moved to another area in this mine.
The east finley elevator was down for more than 30 minuets due to two door switches that were out of adjustment, There was no entrapment.
While scaling the #2 shaft, a lump fell and damaged internal conductors in the trail cable. The mine was evacuated at 1:55pm. Evacuation complete at 2:11pm. The three miners on the #2 cage reached the surface at 2:34pm. The cable was repaired, the hoist tested and miners returned underground on the evening shift at 5:30pm.
An unintentional roof fall occurred on the 1 left section, #3 entry, between spad stations #4431 & 4457. The fall measured 50'L x 18'W x 7'H. There were no injuries, equipment damage or airway blockage.
A METHANE GAS IGNITION OCCURRED ON THE 9 WEST LONGWALL FACE. THE SHEARER WAS MINING ON A HEAD TO TAIL PASS WHEN THE HEADGATE DRUM BITS CONTACTED THE ROCK FLOOR. SPARKS OR FRICTION IGNITED A POCKET OF METHANE GAS WHICH BURNED FOR APPROXIMATELY TWO SECONDS OR LESS.
A possible friction methane ignition occurred on the 1-left off North Mains (MMU 002-0) section. The second cut was being mined out of the 5-right face. The continuous mining machine operator had loaded a shuttle car and was backing up when the cutter bits struck the sandstone mine floor causing a possible friction methane ignition.
Roof fall at station number 14+10 in #2 entry 1st north panel. Fall was 7' high, 20' wide and 65' long. No equipment was involved in the fall, the area was cribbed and dangered off.
A rock outburst occurred on the 4000 level near the bottom of the 40-184 raise. The burst occurred on the afternoon shift when no employees were in the area. When the 43-190 crew reported to their work area on the morning they discovered and reported the rock burst at approx. 7:30 a.m. The outburst caused damage to a 70' x 10' area that will require 10 shifts of repair work.
East 1, 18 block, 6 entry, MMU 007, 4-way intersection. 20'long X 20'wide X 8'high. Support: 6'3/4"" resin bolts, screen, pans, dywidag truss bolts. Fall dangered off, additional roof support installed around fall.
An unintentional roof fall was discovered on the 2 left section, #6 belt #2 return entry, 1 break outby #3956 (spad station). This fall measured 60'L x 19'W x 13'H. There were no injuries, equipment damage or air course blockage.
Fall occur in a 3-way intersection and is within 20 feet of the 100 foot overburden line on the map. Rock is 60 feet long by 20 feet wide and 6 to 8 feet thick and is 4600 feet from the active face. This entry is a branch off of the intake going to the external bleeder. Area is breakered off.
An unintentional roof fall was discovered on the 1 left section, #1 entry, 1 break inby spad station #4468. The fall measured 18'L x 18'W x 6'H. There were no injuries or equipment or airways damage or blockage.
The hoist encoders failed on #1 hoist drive and interfered with hoist operation. The mine was evacuated at 8:40pm and completed at 9:05pm. The hoist encoders were replaced/repaired, the hoist tested and miners returned underground on the morning shift at 1:15am.
A ROOF FALL DURING RETREAT MINING CAUGHT THE HEAD OF THE MINER. WHILE RETREAT MINING UNDER 28' OF DRAWROCK THE DRAWROCK FELL TRAPPING THE MINER HEAD FOR OVER THE 1 HR TIME LIMIT.
Roof fall 130'X20'X5' in the #5 entry inby spad #172. No injuries, ventilation and passage were not effected. Roof support being used in area was 48"" resin bolts.
35'L X 19' W X 7' H. FALL ON #2 SECTION #5 ENTRY NEAR SPAD 2805. JOY 14CM INER COVERED. FALL ON IDLE SHIFT. AGENCIES NOTIFIED.
At approximately 11:39 PM the rupture disc burst on the #3 coal mill system. An overpressure (deflagration) occurred in the coal mill, cyclone, or ductwork causing the disc to rupture. No personnel or equipment damage occurred.
A roof fall has occurred in our Rehab Section (MMU 017-0) in the old ""A"" main track entry. The roof fall measured 18' wide x 15' long x 30"" thick.
FALL ON #2 UNIT, #6 ENTRY. LAST OPEN AFTER THE FALL WATER & SLIP WAS VISIBLE. LENGTH 35' WIDTH 25' THICKNESS 4 1/2 TO 5'.
A roof fall occurred in the 1st Right return in #1 entry just outby #15 crosscut. It was discovered around 2:00 pm on Sunday May 14, 2006. Exact time of fall is unknown. Mr. Art Gore, MSHA inspector, was notified at approx. 2:15 pm. The roof fall measured approx 14' wide, 35' long & approx 8' to 10' high. 1st Right was evacuated & a plan developed to clean & bolt the fall.
An unplanned roof fall occured on the #4A belt towards the #1 working section in the # 3 entry. Fall was 4500 ft.from the working section and measured 75ft long 20ft wide and 6 ft high. No injuries were reported and passage of persons and ventilation were not affected. Entrances to the fall have been breaken off & will rehabilated.
Lightening strike caused fire to office building.
The #8 elevator was taken out of service for a period exceeding 30 minutes to change the suspension ropes. The ropes were changed and the elevator placed back into service on 5/14/06 @ 7:40PM. Escapeways affected were re-routed.
Fall of roof North B section. Continuation of fall reported on 5/10/06. B-Entry North B Section. 17""wide 6 to 8 feet high difficult to determine length. Could be 3 seperate falls or 1 solid fall approx. 18"" long. Grey shale top 6"" resin 12"" cable bolts 20' cable bolts between entry.
A roof fall occurred on #3 unit, crosscut 49, crosscut 5 to 6. The fall measures approximately 45' long, 18' wide and 7' high. The area is supported with 4' fully grouted resin bolts.

A roof fall was discovered measuring 140' x 18' x 12+' high, beginning at the left return at spad #1644, #6 heading going thru crosscut #8 heading. No equipment was involved, no ventilation was disturbed, no injury occured and the fall will not be cleaned up.

the east finley elevator was down for more than 30mins., due to a brake shoe out of alignment. There was no entrapment.
a gas ignition occurred at bottom of clean coal silo at 9pm on 5/10/06. A maintenance man was in process of installing hand rails on cat walk near the feeders. He tested for methane prior to using torch and found 0%. During cuttin process feeder started and he noticed a ""flash"". He stopped cutting and went for water hose, but fire self extinguished.No injury or property damage.
A roof fall occurred 2300' from #2 section in the #1 entry of 10 headgate, inby spade 3719. The fall measured approximately 20' wide x 1 break long x 12' high.
An unintentional roof fall occurred 4, 200'outby the section in the no.3 entry at spad #3542. The fall measured 20'long x 20'wide x 6'thick. This area was developed 1/15/05.
Belt boss found fall in #7 entry from brk 58 to brk 59. Cribbed off and dangered off so no one could enter area.
A roof fall occurred on the working section between 20+20+21+00 in the #7 2nd west entry. The area had been under observation since 3:00 AM. The intersections outby and inby the fall area had already been timbered and dangered out prior to the fall. The fall was combined to 6' high 25' long and 14' wide. The area was bolted with 6"" resin bolts. Water was the cause.
Rock fall occurred 28 breaks inby #2 head drive on East Main in the #6 entry. Fall measured approximately 30' long x 18' wide x 5' high.

A roof fall occured in the 14 north panel, #10 entry, starting in the pillar line inby crosscut #7 and extending half way through the intersection of crosscut #9. The fall measured approximately 85'X19'X5'-9'. The area was supported with 5' fully grouted roof bolts and 9' extension bolts. An undetected slip was determined to be the cause of the fall.
The fall is in 4-B intake, #2 entry, intersection, 2+60. Developed approx. 8/25/05, the fall is 25'long, 20'wide, 6'high. Bolted with 6' fully grouted bolts.
Contract brushing crew cracked a fitting on a 2"" Equitable Gas line. No injury. Equitable contractors repaired line.
Rock fall in 4-A intake #1 Entry at intersection 6 + 65. The fall was approx 6' high 25' long 20' wide development was approx 7/5/05. 6' fully grouted bolts.
Initial mining was OK. The area (#155 x-cut 5 to 6) was bolted with 5' resin bolts & cap boards. A few months later, we timbered the edge of each side of the x-cut. We dangered off the entrances to the x-cut. There was no persons or equipment involved. The area was 40' long, 16' - 17' wide and 4' - 5' high. The fall will not be cleaned up.
THE FALL IS IN 4-A INTAKE, #3 ENTRY, 10 + 30, APPROX. 25'LONG X 20'WIDE X 6'HIGH. DEVELOPED APPROX. 7/25/05. 6' FULLY GROUTED BOLTS.
There was a continuation inby of a previously reported fall that happend on 2/24/06.
At approximently 7:00 am on May 7, 2006 a Dynatec employee noticed a ground fall on the 4835 level of the Deep Star area of the Deep Post Underground mine. It was located in the B-101 heading. This area began working a couple of days earlier but at this time it was still in an inactive area. After it was secured the roof fall continued out into the active work area.
An unknown worker placed some chewing gum in a door elevator switch at the Hart Street portal elevator bottom. The chewing gum caused the switch to malfunction halting the elevator from working. The alternate escapeway was re-routed to the South portal and third shift workers were removed from the mine. Repairs were completed at around 9:20 a.m..
While going down the track, smoke was seen coming out from under the panels. The utility men used rock dust & fire extinguishers to put out the fire. It was determined that the fire started when exposed wires came into contact.
North left section, MMU 004, 5-entry, between 19 & 20 block. Support: 6' 3/4"" resin bolts, screen, pans, brow straps. 17' wide x 20' long x 8' high. Fall dangered off; additional roof support installed around fall.
The aluminum inter-coil connection in the DC hoist motor failed, putting the hoist out of service. 52 miners were on the cage that was being lowered, stranding them 100 feet above the shaft station. The miners were released in about 1 hour when the cage was lowered to the shaft station.
An unintentional roof fall occurred at 11 & 70 entry #2 in the 4/5 intake. The fall did not affect ventilation. It measured 25' x 25' x 6'.
A roof fall occured in the East Mains, #4 entry, half way between crosscut #9 and #10. The fall measured approximately 25'X19'X6'. The area was supported with 6' tension rebar bolts and 6""X6"" square timbers. An undetected slip was determined to be the cause of the fall.
An unintentional roof fall occured on th e1 left mains section, 2 breaks inby spad station # 4468, between the #1 and #2 entries (X-cut). The fall measured 12'L X 16'9""W X 6'H. There were no equipment damages or injuries. This is a 2 way intersection.
A possible methane ignition occurred on the 1-left off North Mains (mmu 002-0) section. The second cut inby the last open crosscut, no. 6 heading was being mined. After a shuttle car, the mining machine was being backed out of the face to set over. The cutter bits struck the mine floor & a possible frictional methane ignition occurred.
A roof fall occurred on the #2 section 1 break outby spad 1028 in the belt entry, 1200ft from the working section. Fall was 18'X 18'X 5'. 6ft torque tension bolts were being used in area. The roof fall was called in 0n 5/5/2006 to MSHA.
A fire occurred at this operation taking more than thirty minutes to extinguish. The fire involved a D11R Cat Dozer, Serial No. 9TR00200, and began at approximately 2:30 A.M. Indications from the mine site point toward the fire began around the engine area.
Unintentional roof fall in #4 Entry @ spad #12701 on the Spruce South Section. 55'(L) x 20'(W) x 6'(H).
Miner operatot was in the process of cleaning up the mining cut, when a methane ignition occured in the face of number 9 entry being mined.
Roof fall on the pillar section on 4 Rt.. One break inby spad #3621- measurements: 45'(L) x 20'(W) x 5'(T).
A roof fall was discovered in the right return on the 8O section. The fall was 160 feet in length and 10 to 15 high and extended from the #23 break inby to #24.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE NO. 1 SECTION, 180'FROM THE FACE IN THE NO.1 ENTRY (RETURN). THE FALL MEASURED APPROX. 30'LONG X 20'WIDE & 4 1/2'- 6 1/2' HIGH. THE FALL DID NOT AFFECT AIR FLOW ON THE PASSAGE OF MEN. THIS AREA WAS DEVELOPED 4/26/06.
Groundfall above the roof bolt anchorage in intersection necessitating rehab.
Loss of power to 30 & 31 hoist at 8:30 pm. Called MSHA at 8:55 pm. Electricians came in & checked power & they found problem & they reset power. Hoist called MSHA back at 10:55 pm to report hoist back in service.
The Enlow elevator was down for more than 30 minutes because of a broken door latch assembly. There was no entrapment.
Lost power to #2 hoist while performing maintenance work. Evacuated mine at 1:35 p.m., evacuation complete at 1:50 p.m. Cause of outage was a tripped circuit breaker. Encountered difficulty opening breaker box for reset so mine was evacuated. Power was restored, the hoist tested and miners returned u/g at 2:00 p.m.
LAMINATED SHELL, ROOF- LOW COVER & UNDER HOLLOW.
Found on weekly examine, a roof fall 6 ft high, 19 ft wide and 80 ft long. Section had been with 5 ft grout and timbers sparingly. We will ramp over the top of it and bolt it with long bolts. 10 + 10 #2 entry Main Section return did not trap passage of miners.
On 5/2/06 a fall of ground occured in ue-162m intersection the ground fall occured after shooting ue162 worth and continued falling on to 8 M back couple of houses it approx arched up about ten feet, going to feet past point anchor.
(1ST OF 2 HOISTING ACCIDENTS THIS DATE.) Hoist car wouldn't come back up due to a faulty 24 volt power supply.
The door switch at the bottom of the elevator malfunctioned, causing the elevator to not operate. Repairman adjusted the switch & the elevator was operational in approx. 30 minutes.
An unintentional roof fall occurred two crosscuts inby SS# 34460 between number 8 and 9 entries of the 001 mmu. The fall dimensions were approximately 40'L x 18'W x 7'H.
(2ND OF 2 HOISTING ACCIDENTS THIS DATE.) Hoist had a small control transformer down.
NORMAL HOISTING WAS BEING DONE WHEN THE OVER TRAVEL CONTACT FAILED, TRIPPING THE BRAKES TO THE SKIP.
A possible methane ignition occurred on the 1-left off North Mains section (mmu 002-0). The second cut inby the last open crosscut (No.6 heading) was being mined. The miner operator was cutting down on the face & when the cutter bits struck the sandstone bottom, a possible frictional ignition occurred.
Unknown cause of fire. It occurred during non working hours and cause was not determined.
Intersection of L-1 Entry in the Continuous Haulage Section 1st Right caved extending through the crosscut toward the belt entry. The cave is approx. 60' long x 20' wide x 4' to 5' high. The area was bolted with 5' resin bolts.
Roof fall inby loading point on #4 unit, mmu-004, #11 entry return last open xc, which was #20xc. 3 way intersection. 18'long x 18'wide & 12'high. This area was roof bolted with 6'point anchor bolts. #11 entry was drove up approx. 60'inby the fall. Bolted & curtained off & #10 entry was mining towards #11 entry at #21xc within 25-30' to blow thru connecting the 2.
Size of fall: length-45ft.; width-19ft; height-10ft. Support-resin, 3 cribs installed prior to fall. Strata: shale, laminated. Rider seam present. Distance above mining seam-10', thickness: 6""-12"". Area will not be rehabiliatated.
A rockfall occurred 1 brk outby spad 114. 62"" to 72"" in height. 65 to 75 long. Crossing midway of 3R to #5 wall. No equipment, no EE's affected.
While tramming outby in the #3 entry, a wood plank, 3"" thick x 9"" wide x 12' long entered the cab of a Fairchild scoop and fatally injured employee.
WHILE PERFORMING MAINTENANCE ON CONVEYOR BELT EMPLOYEE WAS PINNED BETWEEN WA500 KOMATSU LOADER AND CONVEYOR BELT LINE RESULTING IN HIS DEATH.
EMPLOYEE WAS OPERATING A WATER TRUCK WHEN HE LOST CONTROL OF THE TRUCK AND OVERTURNED. THE ACCIDENT IS STILL UNDER INVESTIGATION. THE VICTIM WAS TRAPPED UNDER THE TRUCK. THE VICTIM WAS PRONOUNCED DEAD AT THE SCENE. THE BODY WAS TRANSPORTED TO THE BREATHITT COUNTY CORONER & THEN TO FRANKFORT FOR AUTOPSY.
Employee (EE) climbed over platform guardrail onto raised forklift carrying screens, EE and screens fell from forklift 15 to 17 ft; EE landed on his feet, knees buckled, he collapsed to the ground & apparently one or more screens fell onto employee's body-trunk area abdomen region. Co-worker & injured worker moved screen(s) off of EE & EE scooted himself away from screens.

A master electrician was fatally injured while trouble-shooting 460 volt electric motor. (question #26: experience at this mine TWO hours.)
The employee came into contact with a guy wire, which broke loose and came in contact with an energized condutor on the pole. There was no insulator or proper anchor on the guy wire, this caused the employee to be electrocuted.
COWORKER HAD EMPLOYEE ADJUST THE BRACKET ON THE RETURN ROLLER AS HE WAS HITTING THE BRACKET. EMPLOYEE'S ARM GOT CAUGHT UP IN THE ROLLER AND IT SUCKED HIM THROUGH.
Employee was performing preventive maintenance on a twin screw sand tank. He stepped inside the guarded area while the machine was running. His grease gun got caught in the open gear drive resulting in his hand and grease gun being pulled through the open gears.
Member was shoveling the hi-line tray out on an addcar. He placed his right foot against the end of the guide and it got caught between the guide and the pusher jack.
EE was trying to get clay to flow thru #2 mill direct grind rotary feeder and airslide. He removed cap from pipe between rotary feeder and airslide trying to get the clay to flow. While doing this process the rotary feeder cut off the top of his right index finger just above the join, right at bottom of fingernail. Some bone removed.
Employee's fingers were mashed in the scissor zone of the ATRS while he attempted to locate a blown hose.
Air grease gun had pin hole occur in rubber grease line, injecting grease into right index finger.
Shovelling under belt rollers and felt pain in lower right side. He was diagnosed with a miscropic hernia.
Employee was unloading belt structure when he felt pain in his stomach. He later went to the Doctor and was found to have a hernia.
CRUSH INJURY TO LEFT PINKY FINGER AS A RESULT OF BLUNT FORCE CONTACT FROM A SLEDGE HAMMER WHILE REMOVING CRUSHER WEDGES. LOSS OF DIGIT ABOVE LAST JOINT.
EE was completing repair to skirt board rubber on a converyor. Reinstalled the guards and proceeded to step off backward off the frame and at this moment his wedding band became wedged on the wire screen grard.
EE WAS ATTEMPTING TO OPEN A WALK-THRU MAN DOOR GOING INTO INTAKE AIR COURSE. STRAINED LOWER BACK.
Using a slate bar to pry rock off miner, resulted in a herniated groin muscle. It is felt that this injury should not be considered an occupational injury.
Inj Employee and another employee, were replacing return rollers on m-23 converyor and while trying to break the bolt loose, the bolt broke, causing inj EE to ram his elbow against the trough roller assembly.
EE was climbing down steps of 777 end dump and missed last step.
Cleaning on #3 belt drive. Rock fell striking him on left shoulder.
The roof bolt operator was preparing to install a fly board in 2 cross between 7 & 8 entry. A loose coal & rock rib dislodged, sliding down onto his lower left leg, breaking a bone. Rock & rib measured 10' thick, 14"" wide and 27"" long.
EE stated he was going out on the 4-wheeler transport and hit a bump, hurting tailbone and lower back.
Injured stated he was sitting on the mine floor along the side of the deck of the mobile bridge conveyor he operates when a slab of drawrock fell from between the roof bolts striking him on the head. He was taken to the doctor where he received stitches to his ear and was released.
Employee claims to have injured his back when the scoop that he was operating trammed over a crib block causing him to bounce into the canopy & back into the deck.
Employee was stepping into the elevator inside the Metcalf secondary crusher when she felt pain in her right knee.
Crusher was running & employee checked radiator as the machine was running hot. The radiator cap blew off resulting in a burn on the right side of his upper torso, chest & neck.
employee was pulling on cable. Rolled one eye-cutting bolt was rolling the other when back started cramping-tried to stand up-back popped-could not stand up straight.
EE received burns to his upper extremities due to mechanical failure that occurred on processing equipment.
Employee was mounting a cat DIIR dozer when he slipped & fell to the ground.
Climbing out of grader he slipped and fell from steps to ground about 3 to 3 1/2 feet. Landed on right side bruising ribs and shoulder.
EE was operating a D-11 dozer. Dozer slipped off a rock and he jarred his back.
EE was installing flooring on the 4th level of prep plant when he tripped and fell forward causing him to fall down to the 3rd level. He broke the lower right leg below the knee and cut his left wrist requiring 12 stitches.
50lb bag of dust busted & was going down belt line, EE tried to remove bag & caugt right hand in conveyor.
Employee was cutting a screen with a grinder. The grinder slipped and cut his finger.
Employee operating loader too close to crusher, rock came out of crusher, went through loader side opening and hit employee in face.
Employee was tramming scoop down 1st north travel way & ran over rock in roadway causing him to hit his head, neck & shoulder on canopy.
Employee was helping replace a side panel on the crane, when he slipped off the top of the battery box and fell onto the track of the crane. He then rolled from the crane track and fell to the ground app. 4 feet.
Employee was lifting concrete form panels, strained back.
Supervisor stated the electrician was touching a partially insulated screwdriver to a screw, and it looked like the screw driver touched the panel which created a metal to metal contact. There was an arching, flash and the electrician suffered burns to the right side of his face, right arm and right hand and complained of photosensitivity.
Ee was carring 2 cans of hydraulic oil up flute of steps and felt pain in his abdomen. Diagnosed with hernia. Surgery schelduled 9/11/06.
A large rock came from out of the mountains (not from the permitted area) and hit the truck Ee was driving. This knocked Ee from truck causing only bursies and strains. No bones broken. No cuts, only scratches.
Standing under ventilation tubes while removing them, he cut the wire that was holding them up. Ventilation tube fell on his head.
Greaser was operating a 8000 Ford lube truck returning to parking area at quit time traveling down a App. 6' roadway and stated his brakes went out. He jumped from truck landing on gravel road and fell. He had minor scrapes on R elbow & R side sprained his L ankel. Truck went approx.100' & went up on berm & stopped.
Employee states while testing air conditioning unit on water truck be removed the control unit venting cover and his hand slipped onto the blower motor when he was replacing the cover. Open fracture right ring finger.
Employee bolting top when bolting operator being performed a piece of draw rock fell striking ee on left foot, lower back and top of head.
Employee was loading material and spreading rock dust and strained lower back.
While knocking bratice in tailgate entry a cement block fell on employee's left foot.
Employee was palletizing rock when a slab slid off the rock bank into his foot and caused him to fall into the rock bank, which kept sliding onto his right hand. His right thumb was injured.
Employee was on operator side of bolter, installing a 5 ft. bolt in 6 ft. ht. A piece of drawrock fell onto his left forearm. He was treated at the scene, transported outside, driven to the hospital. Examination revealed a fracture to the left forearm.
EE was pulling jumper cables at the ram car charging station, when he felt pain in his lower back.
Employee was loading loose aggregate into the crusher. It was very windy, blowing small particles in his face & eyes causing eye irritation.
EE was operating a gas weed-eater at the #2 Belt. He reached into high grass to remove a rock and he felt something touch his hand. He did not realize he had been possibly bitten by a snake. He continued on weed eating, felt hand tingle about 5 minutes later. EE went to hospital & was kept for observation. EE is scheduled off until 6/3/06, this case may change to a lost time.
Was walking around feeder when his foot became stuck in the mud, causing him to twist knee. He visited Raleigh General Hospital where x-rays were negative. He was instructed to remain off work for 6 days and referred to Dr. Levins for an MRI. His first shift off due to injury was on 5-31-2006.
employee walked on a motormount catwalk, that was being built. He stepped in the hole for a pump shaft. Sprained right knee.
While throwing a track switch a gil tripped over the switch throw and strained his groin.
EE was working with a sledgehammer in maintance area and smashed his finger.
Employee was lifting/rerailing off track flat when he experienced pain in thoratic region of back.
Injured was walking from track to beltline. Stepping though mandoor, he twisted his ankle, fell and hit knee on rock. His knee was swelling.
Employee was pulling sheets of sheeting up on belt line and his back gave out. Suffered upper back strain.
Employee stated that while loading an oxygen tank into the cage, he felt a sharp pain in his lower back.
EE was standing in the panline on the longwall face at shield #123 insalling the shear cable into the brettbe when a bounce occurred throwing the employee's leg into the spill tray. EE was seen in the ER and released to return to work. His knee continued to bother him for months after and he had his knee scoped on 1/3/2007.
Employee was in the pan line putting the shear cable back into the brettbe. A face bounce occurred and a chunk of coal came away from the face area and pinned his leg to the sigma section of the panline breaking his leg.
EE was changing tailpiece bearing on belt and was hitting the tailroller with a sledge hammer when he injured his lower back.
Employee was a passenger on a tractor that was pushed by a flow of water and sand. The employee struck against the underground rib. He worked the next shift, but experienced continued discomfort. He was examined again on 5/30/06 and an x-ray revealed three fractured ribs. The employee did not return to the remaining shifts in the rotation.
EE lost balance while jack hammering on 62BC11 and fell against a support beam about 3-4' away from his position, bruised right hip
Employee was replacing broken bunker flights when he turned and slipped, twising his back.
Employee was standing at the back of a utility vehicle when a skid steer slowly backed up and pinched him between the two vehicles. The employee's pelvis was bruised as a result and he was taken off work by the physician.
Crusher liner fell on employee's left leg causing pain in left knee. This accident happened on 5/23/06 but employee did not go out on injury until 5/27/06.
This 7000.1 form is being submitted to abate a violation alleged in citation number 7610111 issued on 07/18/2006. The employee was off shift and had left the mine site; he was traveling home on his personal motorcycle. He lost control of the motorcycle two miles outside of the security gate.
Employee was using a cutting torch to remove a broken leaf spring on a golf cart. He was holding spring with left hand and moved cutting torch over left hand. Employee was wearing gloves and cutting goggles.
Twisted right knee climbing out of trackhoe.
Employee was grinding welds on structure and got a foreign object in right eye.
Member was walking towards the #5A tailpiece to clean it out after it had gobbed out, when he stepped on a rock that was in a water hole causing him to fall and twist his left ankle.
EE was being driven to his machine after break. The driver went over a bump in the road and the injured EE began to hurt in his lower back. He was taken directly to the hospital.
Pulling standup, pallet slipped out of hand, came down on back of left leg causing a bruise which became black and blue.
Employee was attempting to hang the battery jumper cables up to the roof after changing the ram car batteries and felt pain in his lower back.
Was removing pipe, cut welds on one end through the other end was free. Pipe was hung, pushed again pipe broke free and smashed thumb on right hand between two pieces. Result was cut and possibly broken thumb.
Employee had fueled loader and was getting down off loader when he stepped on the fueling hose causing his ankle to roll over on its side which broke bone in right foot.
Employee and co-worker were carrying a 800 ton base lift to the face of the longwall when he felt a twinge of pain in his lower back.
While setting timbers, EE stepped back to set wedge and fell backwards over a timber butt. EE fell on his rescuer, which he was wearing on his back, causing injury to his lower back.
Alleged injury occurred when employee slipped and fell as he was dismounting the bottom steps of a rock truck. No witnesses, employee found on the ground with his leg draped over a rung of the bottom steps. No physical injuries were observed. Employee complains of back pain. No medical records to date.
Using power grinder, caught on steel and turned into worker's knee.
The primary plant operator and a co-worker were trying to avoid on-coming fly rock from and overburden shot by entering into the primary MCC #2 building. The injured individual was struck by a piece of fly rock that came thru the south east side of the builing. Blasting services were being conducted y Mt. State Bit Service, Inc. MSHA ID #C4R.
Injured EE along with two other EE's was preparing to overhaul the sand pump. The plate and suction side liner are held in place by 24 bolts and nuts. According to employees it is customary to remove all the bolts and nuts from the plate and side liner then attach a chain to the plate and side liner. In most cases the plate and side liner.
Knocking rock off belt. Rock hit beam the hammer came back and hit right side of head. Bump and scrape on right side of face by eye.
Miner removed radiator cap to check fluid level, when hot liquid splased on right arm and side.
Pulled pallet from stack & received a splinter in his left hand.
Employee was attempting to free a rock hang in a jaw crusher using a hook attached to an electric winch. The hook jumped back striking the employee on the arm and hard hat. That night he went to the local emergency room where a precautionary CAT scan was preformed, there were no injuries, the employee received an injection of pain medication.
Employee lost his footing when he stepped on rocks in the roadway, causing him to over extend right shoulder while trying to stop his fall.
A rounded sheet of metal was measured for a dozer blade. It was chained to some forks. When it was being lowered to the ground when it was about 18"" from the ground it dropped. Because its rounded nature, rather than lay flat on the ground when dropped it rolled over and struch EE on the foot injuring him.
Placing pump on top of channel pump fell pinning his pinky finger on right hand between pump and channel.
Injured was getting a shovel off the wall to get a sample. He got his feet tangled in angle iron that was laying on the floor causing him to fall striking his right knee & hand on the metal grating fracturing his right hand.
Employee was carrying a top belt roller from the track entry (#2) to the belt entry (#1). When he tried to throw the roller through the mandoor, the roller contacted either the door frame or the stopping. Employee pinched his left ring finger between two sections of the roller.
EE stood up from kneeling position and hurt lower back. Started missing work 5/25/06.
Employee and a co-worker were moving a dryer screen from a storage location to the dryer. When picking up the screen, he lost his grip allowing screen to fall. The edge of the screen contacted his right middle finger resulting in a small laceration requiring 8 stitches.
EE was hanging rib mesh turned to his left to step away from bolter. Slipped on loose coal and fell down on left knee. Continued working and then stepping down off bolter, felt sudden pain in left knee. Felt he had worsened injury from previous fall.
EE was helping to pull on belt when the chain broke on the shears. When it broke the EE jumped out of the way, causing him to hit his head on the ground.
employee picked up a heavy piece of steel that was too heavy for him to carry alone and he felt a pop in his lower left side of his back
Employee was taking a coal sample from a moving belt. Shovel struck in belt causing right hand to strike metal tunnel cover.
Original accident occured on 4/29/2006, sought medical care 5/23/2006. The medical provider placed him off work beginning 5/24/2006. The employee was operating a Caterpillar Rock Truck when a broke ball stud caused a break in the hydraulic line causing the engine to catch on fire. Employee jumped from the bumper of the truck causing him to strain/twist his knee.
Loader operator in 980F was moving AB from stacker area to stockpile and hit a bump or dip in surface at 15 MPH, which rocked loader hard, causing EE to jerk in his seat, straining his lower back. EE was wearing seat belt, shock absorber working properly in loader.
A fitting broke in back of a valve bank causing the hose with fitting on the end to strike employee in the chest.
An employee was moving rock across rollers at splitter. Another employee pushed a rock pinning his finger in between the two rocks. There were no broken bones & no stitches were required. The doctor removed the remaining part of his finger nail and bandaged his finger. No further medical attention was required.
While hanging a wing curtain a piece of rock fell hitting employee on back, slid down and struck left foot.
Employee stated he was walking underground, slipped in a wet area and hyperextended his right leg. He felt pain in his right leg and knee. Started missing time on 5/25/06.
Employee was moving the top piece of stone on a pallet to center it (it looked to him like it was going to fall). When he moved it, his thumb was pinched between the top piece of stone and the stone directly under it. His thumb was crushed, the nail is partially ripped off and there was a laceration requiring 5 stitches.
Employee was pulling on the bolter cable to hang it across the entry. As he pulled on the cable he strained his groin causing a hernia. He started missing work on 5-25-06, when he had surgery.

Employee was lifting an overcast over his head and twited his neck.
EE was going into ramp to pick up bucket oversize rock, EE put the Cat in low gear during the initial movement into ramp whe he accidently touched left pedal with released clutch and loader started to free wheel and increased speed and hit pile of rock impact threw him forward hitting his head in corner where windshield and cab meet. Was not wearing seatbelt.
Was pumping up fuel into Gen Set on 05/23/06 when he felt a pain and heard a pop. Went to doctor later that afternoon and was diagnosed with a pulled groin muscle. EE was given a work release on 05/24/06 and released for full duty on 05/25/06.

This report is being submitted to abate citation #7099341, issued on 6/27/06 by MSHA inspector. The managment of the Loveridge mine feel that this is not a work related incident. The EE stated on 6/22/06 while removing miner cable, he experienced pain in his stomach.
EE was descending from the lower (midpoint) of the track area of the D8L Dozer, when he lost his footing and slipped off the equipment injuring his right lower leg. He did not need medical treatment until 5/23/06. He was released for restricted work activity, but there is none available. His first lost workday is 5/24/06.
Employee reported: Installing a roof bolt in drilled hole.This was the rib bolt which required the addl curtain hook over the bolt. When rotating bolt to the roof, the curtain hook started to swival around, ee reached to straighten hook while still rotating and raising the bolt. Tip of right middle finger was caught under corner of pin plate causing laceration
EE was adjusting vent tubing when a piece of rock fell from behind the rib screen and hit him on his hard hat, jamming his neck and back.

Employee was cleaning off the side door and window of the Rock Truck he was operating when the wind blew the door shut on his right hand injuring his middle finger.
Employee was driving mantrip down slope when shovel fell off back of it. He put trip in park, failing to set parking brake, and went back for shovel. Put shovel back on top of trip and noticed trip moving. Tried to regain control by getting in and grabbing wheel pulling mantrip into himself. Resulted in 3 fractures to lower right leg.
After driving dump truck all day, operator reported sorness in his back
He was hanging an angle curtain in #6 intersection and fell off a ladder and hurt his right rib when he fell. Employee went home at 8:00 AM called back to work and said he went to hospital & had x-ray. Said rib was cracked.
Employee was moving the portable stairway so he could grease the top jacks on a 988 F model loader. While rolling the stairway into position, the employee strained his lower back, resulting in a lost time accident.
On Sat., 5/20/06, at approx., 1AM, an explosion occurred underground at the mine. The explosion occurred during the 3rd shift that is designated as the maintenance shift. The source & causes of explosion are unk at this time. Explosion claimed the lives of 5 of the 6 miners. All the victims have been recovered. The surviving miner was taken to hospital, treated and released.
Employee felt pain in his lower back after carrying a 5' piece of stageloader chain from a shield to the headgate.
Was holding shuttle car cable up so scoop could pass under, when a piece of draw rock fell out between rib and bolts. Struck on top of head & neck. Rock was 4' x 2' x 3 1/2"" thick.
EE reports that he was closing a hatch door to #4 cyclone and lost control of the door. It struck his left hand, fracturing L#4 finger.
Employee was pulling into the battery shack on his golf cart when he came into contact with the two wheel utility trailer parked on the back side of the curtain. The curtain was triple layered. He hit the right rear end of the trailer and it rolled upon his golf cart and on top of his left foot.

EE was getting an air reading behind the line curtain in the #4 entry of the 002-0 mmu when the coal rib rolled and struck him on the right hip and leg. He continued working until the end of his regular shift. On Saturday, 5/20, he went to the E.R. where he was treated and released.
EE drove his truck down to the lower bench and stopped his truck 100' behind and to the left of thr 992G Cat loader, in a blind spot wher the loader operator (injured) could not see him. Not knowing truck was in area injured cont. to back drag floor for 2nd operation. Truck driver seen loader backing up darted truck forward in path of loader hitting tailgate of truck bed.
Victim was bolting roof when a rock fell striking him on the chin. This caused a deep laceration of approximately 2 1/2"" wide and 1/2"" deep.
The employee was removing a ladder from the press machine to put near the boom truck. When he tried to do it, the ladder slipped and he jumped out off the truck. He was impacted to his left lateral side and they took 108 stitches.
Upon an investigation the EE was pulling a supply car into the mine using a diesel tractor when a piece of rock fell and hit him. The rock fell from the edge of a half header which was hanging on the roof of the mine. The rock was approx 14"" x 17"" x 2"" & struck EE in the head. The mine roof was about 6 1/2 ft. in height. EE was transported via ambulance to Boone Memorial Hospital.
Employee while slaking a load of lime was splashed with lime slurry resulting in chemical burns. Employee was inspecting the level in the slaker when lime splashed out of the tank and on to her.

He was hauling a load of coal to the feeder when a piece of draw rock, approx. 24"" wide, 41"" long & 3"" thick fell from between the bolts and struck him on the head, shoulder and chest. He was transported to the medical center and admitted for observation. Accident occurred in the first crosscut outby the face.
Employee was standing to one side of 48"" belt. While shoveling material employee was twisting in an awkward position when he felt something pull in the middle of his abdomen.
While carrying detonators on a steep grade, stepped on a small stone & stumbled, pulled a muscle on left leg, behind knee
While moving a loaded railcar injured employee was using the wheel brake and collided with another railcar causing his head to strike the railcar he was standing on. Injured employee was not able to tighten the wheel brake fast enough. Injury is suspected a mild concussion and doctor put employee on lost time starting May 25.
EE was doing his preshift when he turned into the side of the runway when his johnson mantrip hit some rough road and caused steering wheel to turn quickley in a circle causing the wheel to hit his right hand & breaking it.
The employee was injured attempting to dislodge a rock from rock box with a scaling bar. While the employee was attempting to dislodge the rock the belt/roller pulled the bar which resulted in his right hand getting caught between the bar & conveyor.
While pinning top in no.11 entry had 1 row of bolts move up to starting on 2nd row. Rock fell striking him in the face, causing face trauma. Rock 8' long, 41/2'wide and 41/2 to 5' in the middle to the edge.
While installing a prop shaft, cable was making a noise, employee raised up quickly to move and lost his hard hat then struck a piece of angle iron with the top of his head.
Electrician was working on deteriorated overhead electrical line from manlift. A section of line ran over the shop. The electrician exited the manlift onto the roof. While walking on roof he stepped on sky light & broke through the ceiling. He fell to the shop floor below. Safety harness was worn but there was nowhere to tie off.
The employee was working on installing a pipe for a new piece of equipment with another employee. The pipe broke loose and forced the employee off of the ladder from which he was working. The employee came off the ladder in a back and to the left motion and landed (stepped) on a 1"" x 6"" piece of pipe laying on the ground.

THE MINE FOREMAN STOOD ON END OF CART TO BALANCE LOAD. WHEN LOAD WAS MOVED, CART SHIFTED ON FORK OF ENDLOADR CAUSING CART TO FLIP MINE FOREMAN IN AIR AND FALL TO GROUND ON HIS BACK.

EE stated that while walking across the section he stepped on a rock and slipped and fell to the ground injuring his ribs and rght side.

Injured was riding to mantrip when mantrip derailed in switch causing injured to hit back on seat.
The employee was doing some holes with the welding torch in the simplicity. A small part flew and he tried to protect himself and go burned on 2 left fingers.
Employee was installing permanent roof supports in 6 right break. He had just installed the outside bolt in the last row & had swung the drill boom head in to install the inside bolt, when a piece of draw rock fell from the rib striking him on the left leg & ankle resulting in a fracture.
While pulling shields on the F-13 longwall face a rock slid out of the panline and struck employee in the right lower back and he fell onto a shield toe striking his face. Employee sustained a fracture to two ribs.

While ""spotting"" belt structure for a belt move in 12-A section, a top cradle in the scoop bucket contacted the mine floor while the scoop was tramming forward. The structure was thrown out of the bucket. Employee was located 8' in front of & 3' off to the side of the bucket but was struck in the lower right leg by the top cradle. Received 35 staples & 9 inner sutures to close.
EMPLOYEE STATED HE WAS KNEELING DOWN USING A BAND SAW TO CUT UP OLD RAILS. WHEN HE ATTEMPTED TO STAND HE FELT A PAIN IN THE MIDDLE OF HIS LOWER BACK. HE ASKED A FELLOW EMPLOYEE TO ASSIST IN GETTING HIM TO HIS FEET.
EE was resting his hand on the scoop blade, as the operator withdrew the blade, the EE's hand was caught between the back of the bucket and blade.
Picking up starter for 992 loader & pulled muscle in lower back region.
Employee was attempting to get the second drill steel extension into the drill chuck. The drill steel kicked back as he was raising the drill head and hit his wrist. Cause of this injury was safety awareness and drill head not adjusted properly. Compliance with rules/regs was not a factor, PPE not a factor, miner knowledge could be a factor, mining equipment a factor.
The employee was shoveling coal fines away from the rib when he slipped on the wet floor and hurt his back.
Employee was moving a railcar and applying the wheel brake. The railcar did not stop completely when connecting to another railcar causing the employee to strike the railcar's railing. Employee bruised the right side of his ribcage (no fractures). Doctor issued 2-days of lost time. Employee returned to work full-duty on Monday, May 22.
Employee was preparing to drill bolt hole, put drill steel against top of a piece of rock. Rock fell, he jumped backwards out of way of rock, foot stepped on a slick rock lying on bottom, foot slipped twisting his lower back. Started missing work on 5/20/06.
EE slipped on the step as he was entering his truck and fell and hit his arm.
EE was walking across the parking lot when she felt a sharp pain in her ankle. There was no slip trip or fall.She stated her ankle popped. She was transported to emergency room and diagnosed with a fracture. Currently ongoing.

Large rock teetered the loader forward, then the rock fell out of the bucket, dropping the back of the loader. Jarring injury to EE's back, ribs and neck.
Injured was installing monorail bolts. After scaling some small pieces of rock from the top, he began drilling a hole. After the hole was drilled about a foot, a piece of rock appproximately 12""x 12"" fell out and struck him between his right index finger and his middle finger. Injured was wearing gloves. He received seven stitches.
Carrying stopping panels, bent over in low area of the mine, felt pain in lower back.
EE was picking up trash and filling into 4x4x2 cardboard box. When trash was full the injured lifted the box and set it on 2 feet berm to push it over. Apparently he stepped in loose dirt slope causing him to slip lose his footing backward & overextended rapidly. Afterward, EE felt pain in his calf muscle.
Employee was riding in Alpha 14 man diesel mantrip facing to the side (perpendicular) to the direction of travel. He had his feet propped upon the brace. When the mantrip made a 90 deg turn to the rt, the trip was hitting the coal rib. His lt foot hit the coal rib first, he was trying to move his lt foot when toe of his rt foot caught between frame of mantrip and coal rib.
Member was standing on scoop batteries to hang bolter cable in the #2 entry at spad #4531. As he was climbing down off scoop, right knee gave out causing him to fall and twist left ankle. Member continued to work until 5/23/06 when due to swelling, he was told to remain off work to see specialist.

Emp. did work that full shift and worked next two scheduled shifts - he then went to the doctor on 5/17 - and the doctor took him off work.
Shovel bucket on 495BE shovel struck head rack on 793 cat haul truck, jarred truck and injured his back. 5/15/2006 was his Friday; 5/19/06 was his Monday back.
Employee was placing and welding patches on 601 cyclone and strained his back. Employee did not report this injury to his Supervisor till 05/18/06. He is claiming the strain was from an old injury sustained in 1986.

EMPLOYEE DID NOT START TO MISS WORK UNTIL 8/11/06.
Operator of the T542 struck a berm at the King leach 5650 after dumping out,in route back to a shovel.

A belt maint person in the process of winding a 3/8"" wire rope onto a cable spool injured his left foot when his foot became caught between the mine floor and the front edge of battery powered scoop bucket. This resulted in three simple fractures to the left metatarsal bones and he was placed in a walking cast.

Employee was removing belt scraper from conveyor with a wrench when wrench slipped and hit employee between eyes.
Employee was lifting idlers into place on top section tail pulley of conveyor. Injured his back, resulting in back/leg pain, day following the incident. Didn't realize injury immediately and did not notify supervisor. On vacation 5/17-5/23 went to personal physician; had an epidural. Risk Mgmt conducted investigation; accepted claim 6/08/06.

When the blocks creeled, he used his arm to catch himself on a guard. The weight of the body jammed his shoulder. He did not report this accident to supervisor on date of accident.
A piece of stone approx. 1ft x 1ft 6 to 8in thick fell from the nb striking the EE in the neck/upper back area causing a fracture to the posterior spinal process of C6 and abrasions to the neck.

Employee was getting on cage when door of cage went through shoe right behind steel toe hitting left foot. Foot instantly began to swell. Employee was taken to company dr. on 05-13-06 and was diagnosed with a broken toe. Employee is off work.
Left shoulder still bothering EE when turning steering wheel on WA 500. Therapy recommended only.
On 5/15/06 a medical exam of the employee revealed a right side hernia. Employees stated that on 5/13/06 he had been lifting belt structure during a belt move. Surgery to be scheduled
EE was operating truck #107, hualing coal from pit to stockpile area. EE stopped his truck on hualroad to urinate, as he was standing he lost his balance and fell to the ground.
Employee was tramming a shuttle car back and forth from the miner to the feeder when the car trammed over a bump in the road causing an abdominal hernia to surface. This employee had two prior hernia surgeries. The doctor repaired an additional small hernia that was found and not treated when the employee had gall bladder surgery in January 2006.
EE was walking up grade from propane tank to company truck. As he was walking up hill he felt a sharp pain in right ankle and leg. EE went to chiropractor on 5/15/06. *Person had to have surgery on right ankle and began to miss time on 2/14/07.*
Miner was helping loosen an impaler with a pipe wrench, when he strained himself. Miner failed to let anyone know that he was hurt until Wed. of the next week. Company policy states that any injury MUST be reported when it occurs.
Finger was injured while installing a track roller on a dozer.
CM in #4 cut of pillar block loading SC, rib roll pushed EE against SC causing injury to his left side.
Employee felt a sudden onset of mid back pain when moving a shovel tooth from pickup bed. Employee began missing time on 05/13/06 for a back strain.
EE states he injured his right knee while kneeling down to repair a pump, EE states it didn't hurt He did not report it he then went on 3 days off. Returned to work worked 1 day. Next day reported to supervisor he hurt himself on 5/12/06. Co con- tested with WC. On 8/21/06 WC would pay this as on the job injury.
The injured man was descending a short set of steps which he slipped on the rain wetted steps and hit his head. There was nothing broken, but he is suffering from amnesia and will be off work until his amnesia clears.
Pulled muscle in right shoulder while installing turbo in a rock truck when it slid and he tried to stop it. He recently on Aug 17, 2006 was diagnosed with a torn rotor cuff and went off on Aug 18th to prepare for surgery.
Miner was trying to clean a chute from a screen with a hammer. The hammer fell onto the working platform floor and EE jumped from about 3' to the floor. When he tried to get up, his knees could not stand his weight.

EE was installing an air slide in the Packing House. He passed from the stair to the vibrator's structure to reach the chain of the hoist. The vibrator's structure was energized due to a broken ground wire located inside a metal enclosure. EE received an electrical shock when he touched part of the metal structure. The ground was fixed but few wires were broken
Employee stated that he experienced pain in his lower back when he started to bend a roof bolt. A newly rec'd medical report attributes his symptons to a new occurrence instead of related to his previous back surgery making this a reportable accident.

Mechanic was checking cable on shuttle car that was not picking up cable. He took cover off and moved car up because line curtian was in the way. Reel unexpectedly took up tripping him causing him to fall backwards on his rescuer.
Employee was in process of cleaning up shop work area, when he picked up a five gallon bucket of oil that was located beside the coal hauler. When carrying the bucket of oil out of the way, he tripped over a jack stand, when his metatarsal boot caught a yellow tie hanger that was attached around the jack stand causing him to trip and fall.
Employee was walking in a low headroom walkway. He went to lift his head up and hit a pipe causing him to fall forward. He grabbed onto the handrailing. He was treated for shoulder strain and on 5/17/06 he started physical therapy.
Member was standing on second step of a six foot ladder when the ladder broke causing member to fall, spraining his ankle. He was put off work by a specialist on 5/22/06.
Injured was riding in mantrip when mantrip derailed in switch causing injured to hit back on seat.

EE was lifting a phase module to install in cabinet and strained his lower back.
Employee stepped out of grader onto the grader tire. Tire was wet and muddy. Employee slipped and fell to the ground.
Employee was walking along on the Mezzanine Level above the offices when he tripped on floor grating and fell. He attempted to break his fall with his arm, and subsequently broke his right arm.

Bending a loose rib bolt that was sticking out in the tram road and was struck from behind by coal and slate that fell out in three pieces from a corner in an intersection. He was driven to the mine floor. He received 8 fractures to various cervical discs and the blow resulted in a blood clot. Injuries required surgery to resolve.
Employee was marking up belt hanger positions for a belt move while looking up at the top, EE stepped on rock causing him to fall landing on his side causing him to fracture his hip and a fracture to his femur.
Employee was working on a pump drive of OH200 shovel. As he was moving mounting bolts from the pump, the bolt broke loose and employee slipped, hitting his knee against another pump below him.
Doctor prescribed medication, which caused victim to pass out. Victim hit fire extinguisher when falling, breaking one tooth and bruising his jaw. Hit his head on the floor, needing one staple.

Loader operator was instructed by supervisor to push sand toward screen on flat surface. The operator would push forward, and then back up to start pushing material again. Due to inattention, the operator backed the back tires of the articulated loader sideways on a 5:1 slope unbalancing the machine, and the machine tumbled over onto its side.
5I track switch, middle lowboy car jumped track. While jacking car back on track he felt pain in his groin area. 5/22/06 diagnosis was a hernia.
While working on a pet conveyor employee attached a comealong to the head pulley of conveyor. The comealong slipped off hitting the employee in the face. Employee is scheduled for surgery 8/25/06.
Unknown as to whether injury/incident was job related until 5/25/06. Employee was loading a big cable on a trailer. He was the one who had to pick up cable off ground. When he did this he felt pain on right side lower back.
Employee was tightening bolts on motor to clinker breaker, when he strained right shoulder. Employee was placed on light duty.
Pulling bar grating up to elevated level. Workers failed to have guideline on piece, and rope became frayed and broke. This caused grating to fall to ground. The piece bounced off ground and hit employee in calf. A cut requiring 7 stitches occurred. He was off from work from 5/11/06-05/28/06. Returned to work on 5/29/06.
Hydraulic oil sprayed out of the boom lift jack of the roof bolter, spraying oil in miner's face, getting in his eyes. Miner was wearing glasses.
EE tripped on a rock measuring 8""l x 6"" w X 14"" to 6"" thickness and struck his left knee on a cable hanger on the operator side of the miner. He was transported to Clinch Valley Medical Center for testing and was released.
Was trying to remove a bolt, grinder kicked back and hit right cheek.
Victim experienced back pain while hanging a miner cable on No. unit. He began missing work on 2/6/07. An investigation was conducted as to whether the reason for being off was related to this incident. It was determined on 3/19/07 that is was work related.

EE was loading bolting machine with glue when he felt pinch in his lower left back. He thought he would be ok went home came back to work the next day worked approx. 1/2 shift when he began hurting bad enough he could not stand it anymore he was brought out and taken to ER by company offical.
Employee was assisting another with putting a leaf-spring on the water truck when a piece of debris became lodged in his cornea. His eye became very irritated. Debris had to be surgically removed by a specialist.
He was helping move miner cable, picked the miner cable up and felt pain in lower back.
Traveling down haul road ramp into pit 5 and failed to negotiate curve at the bottom of the ramp. Proceeded through berm adn over approximately 12 foot high spoil wall over turning truck on it's side causing injury to head and chest. Appeared to have fallen asleep. During investigation marijuana & methamphetamine were found.
Taking miner cable down from top, cable fell and hit him on the back.
EE was attempting to straighten a piece of roof bolter drill steel that was bent, by lowering the head of the bolt machine on it. The drill steel sprang out and struck EE in the face. EE was X-rayed and had received a fractured cheek bone and jaw.
Employee states as he crawled over the tail brake to install bolts in the tail brake, his foot slipped off the pipe and he twisted his left knee.
Miner walked around shuttle car when he noticed small rocks chipping from the roof. He turned to avoid the falling rock, slipped in the mud and twisted his left knee. Hyperextened his left knee - strained and bruised.

Submitted to abate citation # 8497035 EE was lifting 60 Lb concrete blocks while building brattice. EE continued to work. On May 25, 2006 ee visited a doctor with complaints of back pain. EE was excused from work on 5/26/06 by physician. EE has not bee employed at this mine since Oct 2007
Employee sustained a lower back strain as he was attempting to lift and slide an oxygen tank into back of the yard truck.
He was helping replace a packer. He bent down to pick something up and hit his neck on a super sack fill pipe when he stood up.

Employee was attempting to remove the springs from the coal truck when fingers were caught between the axle and springs.
while stringing lines to work off roof bolt holes, the ee stated that he pulled something in his upper back.
While removing caustic line that goes to the seal tank some caustic got into his boot. While removing protective equipment EE failed to wash his gloves before removing his boots or suit & goggles. Caused chemical burn on right foot. EE told supervisor @ 8:00PM that this happened. Super visor told him to remove boot.EE told supervisor not a problem only small amount.

Employee was bending a roof bolt when he experienced a hernia of the groin that required surgery.
Employee was tramming the scoop to retrieve supplies. The scoop stopped tramming, so he re-cycled it. When doing so the scoop jumped into tram hitting a hole in the roadway bouncing scoop in to rib. EE injured his back and right side. He visited Raleigh General Hospitial, Where he was instructed not to return to work until 5-15-2006.
EE alleges to have injured his back when he moved a Heintzmann Jack. This alleged accident was not reported until the end of his shift on 5/10/2006.

EE (Mechanic) was wworking underneath water truck after he had crawled from underneath of truck he stood up and his left knee flew out of place. This incident did not require First Aid or Rescue Squad.
Employee was riding in a track mounted mantrip when the operator stopped suddenly. When the mantrip stopped, it was struck by a locomotive. This resulted in the injured having pain in his back & neck.
While re-installing the relay bar pin to the pan line. A crib block was used to apply pressure to the relay bar, assisting in aligning the holes. The crib block flipped out and the shield rocked forward striking the individual's hand, causing a fracture to his right little finger.
While rock dusting dust got in eyes.
EE WAS PULLING A SET OF BATTERY LEADS FROM ONE BATTERY TO THE OTHER.
Complacency walking down set of narrow steps that the EE had traveled numerous times before. This time his foot slipped off the front of the step, causing the injury.
The welder was prying out a shovel cable that was caught between the crowd bull-gear and it's guard when it broke loose. The cable struck the welder's left eye, face and his left little finger. The impact from the cable also caused him to fall backwards onto the crowed track below.
Employee was changing oil in the cone. As he removed plug the drain valve also unscrewed and hot oil splashed onto two fingers of the left hand burning him.
Around 9:30 pm EE stepped in a hole in the inby intersection of the #5 entry and twisted his knee. He finished the shift but as the shift progressed, the swelling got worse and so did the pain.
EE was desending the ladder. At mid-point when he stepped down, his weight shifted pushing the ladder to the right. His right foot became entan geled in the step of the ladder.
Employee was shoveling under boom of his shuttle car and pulled his right shoulder out of socket.
Employee was moving pump and stepped in hole. Strained knee. Began missing work 5/8/06.
Employee stepped on a section of grating at top landing of double bin transfer point, twisting his right knee straining it (employee started missing on 5/24/06).
EE was installing a hood lift assembly on the 988G loader. After connecting the pin & spring clip to the cylinder end of the assembly & the hood, he proceeded to place the clip & pin on the rear assembly end. Pin would not go through. EE attempted to align the pin by activating the hood to relieve pressure. The hood raised, the pin popped out & his hand was caught in the machine.
Ledgeman fell when a granite block broke causing him to fall on his shoulder and rolled under a scrap pan and hurt his knees also.
