an unintentional roof fall was discovered on the c-panel 1 left mains at spad station 4412. The fall measured 20'l x 20' w x8' h. #4 entry (belt)
An unplanned roof fall occurred in the #5 entry during pillaring. Equipment involved-14/10 c/m. No injuries were reported, ventilation was not affected, passage of persons was not affected. All entrances to fall have been breakened.
The slope car serving the #1 portal was taken out of service when the ethernet cable used to transmit communications malfunctioned. The car was unavailable for use for two hours, from 3:15am until 5:15am.
While operating a D11R Dozer an employee noticed some smoke on the right side of machine and discovered that the machine was on fire. He activated the fire suppression system and said that it seem that only about 1/2 of it went off. He activated the system on the rear of dozer and used a 10 lb. extingusher but called local fire dept. to use foam and about 7:00 pm got fire out.
A roof fall occurred at 15+60 in the 4th south panel. The fall was 80 x 19 x 10. The area had 5' glue bolts & 12' cable bolts in it. The belt only had 5' glue bolts. The fall was 3800' outby the working faces. Fall will be cleaned up.
A roof fall occured in the #11 seam, #1 unit, crosscut 16 between #2 and #3 entry. The fall is 100' long, 18' wide and 10' high.
an unintentional roof fall was discovered on the c-panel off of 1 left mains. The fall measured 80'l x 8'h x 18'w. This fall is located 1 break inby spad #4435 in the #4 entry at the c panel. This panel is located inby the section loading point. There were no injuries or equipment damange.
An unintentional roof fall occured on the 4th Main West between #3 and #4 entry at #198 cross cut.This roof fall was supported with 6' and 10' point ancor roof bolts. The roof fall measured 36' long x 18' wide x 14' high and the roof fall will be cleaned up.
An unintentional roof fall occurred in the South East Main return, x-cut 28+00 Entry #6-entry#7. The cause of the roof fall was laminated and unconsolidated shale at and above the anchorage point. The area had 6' fully grouted rebar bolts and timbers installed for roof support.
Fell 12 brakes outby face at #4 entry at spad 1365. 20' Wide X 40"" long X 5' thick. Neutral location.
The PC board on the DCS 500 drive went down on slope car drive. We ordered the part from Frontier-Kemper (who installed it), and it arrived at 2:45 AM on 4/27/06. Repairs were finished at 3:30 AM and a spare was bought also to prevent this problem again.
A roof fall occurred on the #1 unit, crosscut 6, #4 entry - intersection at spad # 15059. The fall measures approx. 25 feet long, 25 feet wide and 9 feet high. The area is supported with 4 foot fully grouted resin bolts.
A rock fall occurred ap[prox. at 5:00 am on #1 unit in the intake from #8 entry to #6 entry blocking the intake. Some intake was open in #8 entry from rib approx 5 feet. Rockfall was 180 X 6 X 19 feet, 40 feet outby working faces.
The Hoist for the Man/Material Shaft for the Gateway Mine was unable to be operated due to a mechanical condition. The employees were called out of the mine and the mine idled until repairs were completed. The hoist was inoperable from 2:55PM until 4:25PM.
An unplanned roof fall occured 1st right mains (Bull Push) at spads 4635 3 breaks inby 8 tail in between 7 & 8 entry fall measured 56'long 20'wide 6'to8' high.
At approximately 4:40 p.m., supervisor noticed a smoldering burn hole in the flex coal pipe which connects the #2 kiln and burner pipe. He immediately used a fire extinguisher and extinguished the fire. The system was shut down and the pipe was replaced. System was restarted at 6:20 p.m.
A Contractor employee was striking an oxy/fuel torch when an unplanned ignition of acetylene gas occurred near the #2 clinker cooler. There were no personal injuries or equipment damage. The contractor is Huelsman-Sweeney Construction.

A roof fall occurred on the 2L4W panel at crosscut 35 to 36 in entry number two. The cause of the fall was weak laminated shale often referred to as stack rock. The fall was located in the return aircourse of #3 unit.
Unintentional roof fall on #12 beltline between breaks #94 and #95 Fall was 20'widex20'longx10' high.
An unintentional roof fall occured @ 4 + 80, Entry #8 in the 3A Panel off of the Main North (Return Aircourse). The fall did not affect ventilation, passage of men or cause personal injury.
A roof fall was discovered in the 2 West #3 submains #5 entry at 30 xc.The fall measures approximately 25 feet long, 18 feet wide, and 6 feet high. The area is supported with 4 foot fully grouted resin bolts.
Unintentional roof fall 3rd south submain 1580' mark #4 entry 3SSM. 18' long, 35' wide and 10' high. Bolted with 6' tension rebar bolts and 8' tension rebar bolts. Mined approx. 3/21/06.
At 10:32 am the power to the #3 main hoist was lost due to high winds knocking a tree top into the power lines. 56 employees stopped work and stood by until the utility Co. restored the power at 11:55 AM. No accidents or injuries as a result. Bosie MSHA office notified at 11:05 AM.
Grass fire along the railroad track; no damage to equipment.
Roof fall 18'wide, 100'long and approx. 10'high in #3 belt entry and traveled north for 100'. Roof was laminated.
An unintentional roof fall occurred in the 8I section, on the future longwall face. The fall measurd approx: 300' long X 20' wide X 12' high. No injury or equipment damage.
The #6 Portal elevator stopped while traveling downward. It was about 3/4 of the way down the shaft with 9 EE's on board. The brake holding failure circuit became activated and after investigating the situation, the mine surface electrician reset the circuit, correcting the problem. From 11:25pm to 12:01am.
Repairs were made on a piece of equipment during dayshift. At the end of the day shift an oxygen bottle was left in the scoop bucket. During the 2nd shift the scoop was used to clean a feeder approach. The oxygen bottle was pushed into the feeder and the pick breaker ruptured the bottle causing a sudden release of gas. The bottle was held in place by the pick breaker.
An unintentional roof fall occurred in the #8 return air entry 2, 900' outby the active section at spad # 3485. The fall did not affect passage of men or air flow. The fall measured 25' long, 20'wide and 4'-6' high.

Kiln stopped due to disruption in raw material feed, coal mill stopped. smoldering palverized coal made its way to the baghouse. Potential ignition source was tramp metal in coal mill, CO2 inerting was effective until CO2 level in storage tank became too low to start mill system. High O2 atmosphere in baghouse due to mill off-line & lack of CO2 led to need to stop smolder w/water.
Bad top door landing switch. Elevator was repaired and put back into service at 2:52 a.m.

A roof fall was found out by the working section at 8+20 #5 to #7 entries of the 2nd west mains. Ventilation was disabled and then immediately reestablished by construction of new stoppings. It was originally supported with 6in resin bolts & secondarily supported with 10' cable bolts. Fall was 120'L, 18'W & 10'H. Upper block coal seam was cause.
A roof fall occured in the south sub main, #8 entry, #11 crosscut, 730' station. The fall was approximately 25' long, 25' wide and 8' tall. The area was bolted with 6' and 8' point anchor bolts. The fall did not affect ventilation, travel, cause injury or damage to equipment.

Strata consisted of shale. Cribs, timbers and 10'cable bolts were used in addition of 6' fully grouted torque tension bolts when intersection was driven up. Fall was 40' in length, 20' in width and 7-8' in height. Located in #1 entry (return). Return was re-routed into #2 entry.
A roof fall occured in the return entry outby #2 section located inby spad # 1388. This fall measures 25 foot long, 25 foot wide and 6 foot high.
The roof sagged against the frame of the roof bolt machine in the #2 face for a span of approx. 12' in length and 12 to 14' in width. An area of approx. 8' X 19' wide of unsupported (unbolted) roof fell directly in front of the machine at the face. (After the initial call-in report, further investigation revealed this was not a reportable incident.)

Front end loader operator was removing rock from the face. He noticed a large rock in the floor of the room. The operator checked the rock for any evidence of detonation cord and upon observing none, proceeded to use the bucket of the front end loader to ?flip? the rock over. When the operator ?flipped? the rock, he noticed an explosion.
The No. 1 Slope hoist wire had incurred damage from the inside as determined by a nondestructive test, resulting in the hoist being down for more that 30 minutes while the rope was being replaced. MSHA and OMLS were notified.
A roof fall was discovered between the 8O and 8P recovery faces in the #3 entry. Beginning inby the #1 crosscut. The fall length is 90 feet long and 20 wide. The fall will be breakered off.
A reportable roof fall was found in 19 Tailgate, entry 3, crosscut 14. The fall was approximately 20 feet in length, 9 feet high, and 16 feet wide.
A roof fall was discovered adjacent to 7A belt line to 2 breaks inby the head drive. The roof fall measured 5 ft high and 18 ft wide and 30 ft. No injuries were reported as a result of this roof fall.
An un-intentional roof fall occurred in the LOB intersection of #2 entry to #1 entry near survey station 4355. The fall will not be cleaned up at this time. Fall dimensions are 19' wide x 50' long x 6' to 8' thick. 4' and 6' resin bolts were being used at the time of the fall. Fall was reported to the Pineville field office.
On 4/17/2006 at approx 9 am an unplanned roof fall was found at the old 1st N-W mains Spad No. 9162 in #9 entry. The fall does not impede travel or ventilation and has been hazard danger off.
Roof fall found on 4/17/06 at 8:00 am. Fall occurred between 4/15/06 and 4/17/06. Roof fall was in #4 entry near spad #80. It was approx. 55 ft. long x 20 ft. wide x 5 ft. thick. Roof fall was approx. 8500 ft. away from working face. Area was developed in 1978 and supported with 48 in. resin bolts. Area was breakered off and dangered off.
Between 1+4, 2+10 #3 entryon submain southeast. Fall measured 60ft long, 18ft wide and left high, area was bolted with 5 ft fully ground bolts. Developed 10/20/03 and we will not clean up.
An unintentional roof fall occured at the Galatia North Mine at the #3 Entry of the 13th West longwall, crosscut 27. The fall was approx. 18 feet long, approx 20 feet wide and approx 14 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up.
Approximately 80' past the Deep Star cross cut wash bay, in the direction of the Deep Star Mine the left rib (east rib) springline area sloughed in. The ground fall was approximately 20' long and 18' high. Utilities were dropped to the sill with minor damage. The ground fall resulted in blockage of the Deep Post secondary escapeway, loss of ventilation, and loss of water.
Fall of roof entry west section @ SS 39&20 approx. 35 lg 17 wide 5 1/2 ft high 48' resin grouted rods. Grey shale sandstone.
High temp alerts were going off for Kiln 3 cooler tunnel. It was during shift change and the 7 pm conveyor man was called to go check it out. They saw smoke from the building and called fire dept for help at 7:02 pm. MSHA was notified at 8:15pm. Belts 57-811 and 57-812 were burned. Fire dept was cleaning up at 8:30pm. Unsure exact extiguish time.
Production hoist was disabled due to a short circut condition on the incoming power feed.
A laminated shale rock fell out on belt line 10' inby spad # 1031.
A roof fall was discovered in 2East that was approximately 20' wide x 20' long x 7' high. The fall will not be cleaned up.
Rock is 60 ft. long, 20ft. wide by 6-8 ft. thick. Fall occurred in a 4-way intersection. This area was developed in April, 2005 and was supported with No. 5 x 48 inch full grout bolts with 8x8 plates. A coal rider seam is present at 8 ft. and is wet.
The distribution of excess weight onto the longwall face near the tailgate and adjacent tailgate pillars, preventing passage.

An unplanned roof fall occurred on the main north #1 beltline at 58 crosscut. There were no injuries or entrapment. The fall was 20' wide x 25' long x 6' high. The fall was above the anchorage zone.
A roof fall occurred inby the dump point at crosscut 14+60 in #4 entry of the second west mains. The fall was 25' long, 18' wide, and 11' high. The area was initially supported with 6' fully grouted resin bolts. The area was secondarily supported with 10' cable bolts. Localized weak sandy shale was determined to be the cause.
Loss of office due to fire. We believe that this fire does not meet the reporting criteria in Part 50 for the following reasons: 1) The fire was extinguished by the time it was reported to Florida Rock personnel.2) No one was injured or even present during the fire. 3) The building was well removed from the active mine site.
The elevator doors would not open or close properly. The doors were adjusted and elevator put back in service at 09:02 am.
1 Main East in half the pillar line between crosscut #16 and #17 and extending through half the intersection of crosscut of corsscut #17 in #3 entry. The fall measured approximately 30' x 19' x 9'. The area was supported with 6' tension rebar bolts.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 1ST WEST SUB-MAIN OFF THE 4TH N. SUB-MAIN IN #9 ENTRY AT #18 CROSS CUT. THIS ROOF FALL WAS LOCATED IN AN INTERSECTION & WAS SUPPORTED WITH 6'POINT ANCHOR ROOF BOLTS. THE ROOF FALL MEASURED 20'LONG X 18'WIDE X 9'HIGH & THE ROOF FALL WILL NOT BE CLEANED UP.
AN UNINTENTIONAL ROOF FALL WAS FOUND BY THE PRESHIFT EXAMINER. NORTH SUB-MAINS XC 20, E#3. THE FALL WAS APPROX. 4.5-5' H x 20' L X 20' W.SEVERAL 5' BOLTS IN THE CENTER PULLED OUT. AREA WAS BOLTED UNDER THE OLD PLAN USING 5' RESIN BOLTS. THE MINE HAS SINCE BEEN USING 6' RESIN GROUTED BOLTS. AREA WAS DANGERED OFF, TIMBER SET. BEACHLINE SUMP EP'S CAN STILL BE EVALUATED.
Rock outburst occured in the 43-190 west slope. 2 miners ans shift supervisor were in the area at time of incident. 1 miner was working at the area burst occured.He received minor scratches and bruise on right leg. An area roughly 8-10' long, 3-4' deep and 4-5' high loosened by the burst

A roof fall occurred measuring 5x5x12 beginning 30' outby spad 47 on #1 belt 4 breaks inside. #1 belt was involved, no ventilation was disturbed, no injury occurred, and the fall will be cleaned up.
Mine fire occurred on P19 belt, between the new crusher and the sizer/transfer area. It started sometime between 4 am, 4-9-06 and 6 am, 4-10-06. The mine was empty at that time. Our investigation began @ 7:30 am. MSHA was notified by 9:30 am as a precaustion. A fire wasn't discovered until later after lunch time.

INTENTIONAL ROOF FALL 001-0 SECTION WEST B ENTRY AT SS4 +95. APPROX 25FT LONG, 20FT WIDE, 7 FT HIGH GREY SHALE 6' RESIN BOLTS 12' CABLE BOLTS; NOTE; X CUT & AREAS INBY THIS HEAVY AREA WERE POSTED & RE CABLE BOLTED. WAS ALLOWED TO FALL TO RELEIVE STRESS
HOIST OPERATOR OBSERVED THE EAST HOIST MOTOR TRIPPING. IT WAS DETERMINED, AFTER TROUBLESHOOTING, THAT THERE WAS A BAD MOTOR BEARING. THE MINE WAS EVACUATED. THE EAST MOTOR WAS ELECTRICALLY REMOVED FROM THE CIRCUIT & THE HOIST WAS SAFELY TESTED SO IT COULD BE USED AS AN EMERGENCY MANTRIP (2 ESCAPEWAYS). THE REGULAR ACCESS HOIST WAS NOT AFFECTED. UP AGAIN AT 4:20PM.
Roof Fall is appr 30' in length and 20' wide by 4' thick in the number 4 entry in the intake 2nd crosscut underground. The escapeway has been re-routed thru another crosscut and entries dangered off going into the fall area.
A fire started in the production shaft located at the Elkhart portal. 3 men working in the shaft at the time escaped on elevator. Fire Dept arrived on scene & worked to extinguish fire in shaft. Fire extinguished at 2: PM confirmed by inspection of shaft bottom. Cause of fire under investigation. No injuries. Property damage has not yet been determined.
Roof fall in #2 entry 18' Long, 5' Wide, 42"" thick. No injuries were reported. Roof fall occured 640' Outby section, reported to Lincoln Selfe on 4/7/2006 at 5:20 PM. Fall will not be cleaned up. All approaches have been timbered off. Ventilation and passage of people not affected. Area was supported with 42"" fully grouted bolts with 16' J-channel.
A roof fall was discovered 60' inby station 3654. The fall was 20' wide x 20' long x 7' high. The fall will not be cleaned up.
A roof fall was found during the weekly examination of the return air course. The date and time of the fall is unknown. The fall was located in the Main North return just inby spad #34+39 in the #7 entry and in the x-cut at 35+29 between #7 and #6 entries. The fall measured 90'x18'x6' and 40'x18'x6'. The area seperating these two points was cribbed.
Contract employees were at JM Huber - Wrens 17C mulching the dam with hay blower and tractor. It was windy and some of the hay blew under the tractor and came in contact with the exhaust, causing the tractor to catch on fire.
A roof fall was discovered approx. 2 breaks inby survey station #187 in the no.7 intake entry during a preshift examination. The fall measured approx. 30' long x 20' wide x 4'-5' in thickness. The fall was in a slip form and was composed of sandy shale. 42' fully grouted resin bolts were installed in the area.
A roof fall was discovered in the crosscut between the #4 and #5 intake entries at survey station #28. The fall measured approx 40'long X 15'wide X 4'thick and fell in a slip form. 36"" mechanical roof bolts had been installed in the area.
UNPLANNED ROOF FALL ABOVE ANCHORAGE ZONE IN 7TH NORTH WEST BUTT #2 ROOM OUTBY SPAD #1692, TWO CROSSCUTS IN AN INTERSECTION. APPROX. 20'WIDE X 30'LONG, FELL ABOUT 42"" TO 48"" HIGH. ANCHORAGE ZONE WAS 42"" FULLY GROUTED RESIN BOLT.
A ROOF FALL OCCURRED ON THE MAIN #2 ENTRY, SPAD #2227, 7800'FROM ACTIVE FACE. THE FALL MEASURED APPROX. 40'LONG X 30'WIDE X 5'HIGH.
A roof fall was discovered one break inby survey station #6970 in the 16th developement during a weekley examination of the area. The fall measured approx. 20' long x 20' wide by 4'- 5' thick . The roof was composed of laminated sandy shale and had been supported with 36' fully grouted resin bolts.
A roof fall occurred 1 break inby spad 152. The fall was approximately 35 feet long, 18 feet wide and 6 feet thick.
A fall of roof rock occurred in the No. 2 to No. 1 intersection near spad location 2032. The fall measured approx. 25' long x 18' wide x 4' thick. The area had been supported with 42"" bolts. This area had previously been cribbed off. There were no injuries, no effect on ventilation and no equipment involved.
An unintentional roof fall occurred 120' outby section in #9 entry of old panel. The fall measured 140'long, 20'wide and 4'-6'thick.
The dry grass along the public highway caught fire and spread to the mine property. The fire was reported it to mine management. A dozer cut a fire break through the woods and the company water truck was utilized to prevent the fire from becoming a real hazard. The local fire department arrived and put the final touches on extiguishing the smoldering remains of the fire.
An unintentional roof fall occurred in the #9 return entry 240' outby the face. The fall measured 25'long, 20'wide and 4'-6'thick.
The accident was a roof fall in the #3 Entry of the active working section MMU-001. The 12-12 Remote Continious Miner was being operated at the time of the roof fall. The miner was taking his second cut out of X-cut 3 to 4.
A roof fall was discovered in number 4 entry at spad 419 to 415, the roof fall measure 10ft high 18 ft wide and 50 ft long. No injuries reported.
A power outage at the substation caused a power failure to the Nordberg Hoist and entire site. When the back-up generator/emergency power was started the push button for the circuit breaker failed to close the circuit breaker so no power was being supplied to the workdeck winches used to evacuate men. Ventilation was interupted for more than 15 minutes.

A roof fall was discovered in the 2 north #3 mains, #2 entry, crosscut 11-12 the fall measures approximately 70 feet long, 18 feet wide, and 7 feet high. The area is supported with 6 foot fully grouted bolts and 12 foot cable bolts.
Broken cable pulley for Main Portal Elevator doors. Elevator was repaired, inspected and put back into service at 10:25 a.m. on 4/5/06.
MINE SHUT DOWN ON FRIDAY 3/31/06 FOR THE WEEKEND. CAME BACK MONDAY 4/3/06 AND ON PRE-SHIFT FOUND A ROOF FALL IN LAST OPEN LINE OF BREAKS. THE FALL WENT FROM INTERSECTION OF #5 ENTRY OVER TO THE EDGE OF #10 ENTRY.
Intersection dropped out 1 break outby SS 1043 in #3 entry. Intersection dropped out 5'thick 18'wide 34'long. Fell from #2 toward #4 crossways. Reported to MSHA as soon as it was found on Monday morning. Called out at 7:50AM by foreman.
Cleveland Public Power lost an electrical feeder that supplied most of the mine electricity. When the power went out the service hoist was lost. Underground employees were evacuated out the production shaft. Power was restored at 11:05 am and employees returned to the mine. MSHA field office supervisor, Bob LeMasters, was contacted by phone within the Part 50 time limits.
An unplanned roof fall occurred in the #7 entry at spad #16833. There was no active face due to section in process of moving. Ventilation and passage of persons was not affected. There were no injuries reported. Length was 350' x width 18' x 6'-8' height. Equipment involved - roofbolter.
Shaft was down 12 hours due to damage, inspection, repair, and subsequent investigation.
A roof fall measuring 17'L x 17'W x 7'H occurred in the #2 entry off No. 2 Section at Spad No. 7503, No. 4 SE Mains. Area was bolted with 5' bolts.
After mine examination and power restored (power company had power off last shift) found electrical problem with hoist. Blown fuse was replaced and hoist was put back in operation at 8:45 a.m.
An unintentional roof fall occurred at the Galatia North Mine at the #1 entry of the 13th west longwall at 6818', pot out between tip of headgate shields and crusher approx 15'thick, approx. 18'wide and approx 20'thick. The fall did not affect ventilation. The fall will be cleaned up. Fall #23 D
While the fireboss was making his pre-shift examination, a roof fall was discovered in 4 North Mains. The fall was located between No. 5 and No. 6 entries in the crosscut near spad No. 832. The fall was located 500 feet about the working section and it did not affect travel or ventilation.
A roof fall was found by the pumper in the return at 7+80 #1 of the first west entries. The fall was 30' long, 18' wide, and 12' thick. The area was initially bolted with 6' resin pins and was secondary bolted with 10' cable bolts. Water in the roof was determined to be the cause.
The Cedar Creek Service Elevator was out of service from 11:37pm on 4/1/06 to 1:20am on 4/2/06 due to the over speed tripping on the elevator in the up run mode. Bagby elevator company came and changed out they tacho-meter and tested the elevator and then it was put back in service.

D-78 cat D-11-R dozer caught fire from a ruptured hyd. hose spraying on the turbo. Fire burned longer than 30 min's. No one was injured. The fire was reported and investigated.
Utility operator was preparing to wash out the rail scale. The loader operator was weighing the loader on the scale. The loader operator announced on the radio that he was moving the loader off the scale. He slowly moved forward when he observed the utility operator on the ground. The utility operator apparently fell and was crushed by the front left tire of the loader.
Two men were drilling bottom line when a seam in the next cut back gave way. The piece of stone that broke loose was large enough to crush both men. One man was fatally crushed and the other is in critical condition.
Employee was found pinned between the #4 mobile bridge unit and the section lo-lo conveyor belt structure. He was apparently troubleshooting a report of weak tramming on the left side of the mobile unit. There were no witnesses to the accident.
Employee was walking to his shuttle car when draw rock fell from top pinning lower half of his body underneath. Rock was 20' long, 86"" wide, 6.5"" thick.
Employee was placing wire around a pallet of stone. He disreguarded the backup alarm of a Case 95XT skid steer loader that was moving in reverse. The employee disreguarded his safety training to yield the right-of-way to moving equipment. As a result, he was struck by the loader and died from his injuries.
Employee received fatal injuries when he became caught in the dolly of a mobile bridge system.
BEAMS AT INTERSECTION FELL DROPPING BEAM ONTO MOTOR CAUSING IT TO ENTER MOTOR DECK WHERE EE WAS SITTING DRIVING MOTOR GOING INTO BODY CAUSING DEATH AFTER A SHORT TIME.
Employee crawled 16 feet under primary and was entangled in tailpulley.
The employee suffered a fatal electrocution injury while engaged in positioning the electrical MCC building. The boom truck became energized when contacting overhead power lines.
Employee was helping remove an overflow pipe when his finger was caught between the pipe and the cyclone.
EMPLOYEE WAS DRILLING HOLES IN ROOF TO SHOOT BOOM HOLE. HE REACHED ON THE SIDE OF THE BOLTER TO GET A PIECE OF BOLTER STEEL. HIS LEFT HAND WAS STILL ON THE BOLTER ARM & HE REACHED WITH HIS RT. HAND. HIS SHIRT POCKET CAUGHT THE UP-DOWN LEVER FOR THE BOLTER ARMS CAUSING THE ARMS TO GO DOWN & CUT HIS FINGER.
Miner foreman crawled out into intersection to look, swag was in area, ram car came over swag. Foremen did not see car. The car artickiled down, bucket end hit foreman's right leg.
While roof bolting, EE finished drilling a hole and began to drop the head. As he dropped the hesd, he grabbed the drill steel and the head shifted and caught the tip of his finger of his left hand little finger between the head and drill steel resulting in amputation.
Ignition switch on pump engine not working properly. EE tried to turn crankshaft to get flywheel & starter in time. He grabbed fan with l. hand & pushed fan belt in with r. hand. The engine started with key in off position & pulled his r. hand fingers (index & middle) in between fan belt & crankshaft pulley. Severed fingers (index: middle phalange; middle: distal phalange).
EE was removing a wheel chock that was used to fill in a ditch that caused the buggy to get high centered, when removing the last piece of timber from the back wheel, the unit moved forward 2 - 3"" causing his right thumb to be pinched/caught between the buggy rear mancarrier & the wheel chock pinching the tip of the right thumb off.
Employee was removing bucket from excavator, when pin that other employee was hammering out suddenly came out, crushing the tip of employee's middle finger, of his left hand.
EE was standing at the chain link fence of the R-8 belt drive. The drive chain broke hitting him on the right hand, amputating his right finger at the firsty joint.
Employee was kneeling by operator's compartment repositiong scoop with left hand resting at a pinch point area. As he steered the machine his finger was pinched between frame and battery end of scoop. His finger was crushed. Doctor amputated little finger to proximal joint.
WHILE USING A PRYBAR TO DISLODGE A ROCK FROM A MOVING TAIL PULLEY, THE BELT GRABBED HOLD OF THE PRYBAR PULLING EMPLOYEE'S HAND INTO THE MOVING BELT, SEVERING THE TIP OF HIS FINGER.
Employee states while walking and carrying pipe, he slipped and fell to the deck. Pipe fell onto his right ring finger.
EE was positioning a stone on the bottom teeth of the splitter. The scrap tray popped up and pinched his left pinkie finger between the tray and the stone. *Partial amputation of left small fingertip. *Restricted duty.
EE was installing rof bolts at Face # 3 entry was getting drill steels out of drill chuck. He was going to turn on the rotation but pushed the wrong lever & drill pot went up and pinned his left index finger to the roof.
Employee was carrying a roller frame and tripped over another frame on the ground.
EE dislodging object in beltline using pry bar. Pry bar contacted moving part, pinning EE's thumb & hand against frame of machinery, causing severe laceration to right thumb & minor laceration to ring finger. While trying to remove trapped right hand, left hand became injured by pry bar; minor lacerations to left middle & ring fingers, with dislocation to left middle finger.
He was using a ratchet to remove the bolts from the pads of a TRS cylinder. The ratchet slipped and he felt pain in his lower back.
Employee was running buggy when a tire blew out on the buggy & the deck hit the bottom. This threw him around inside the buggy bruising his rt. knee & left rib.
Employee was holding a 6"" x 16"" roof plate in line and caught his right index finger between plate and the roof.

Employee was dragging curtain through #2 Hd intersection. Drawrock fell causing injury to left ankle. The fall caused laceration over right eye and nose. Stitches were used over the eye and x-ray of ankle was positive.
Employee was working on scaffolding when while moving material, he fell off end and struck right side.
Operating an End Dump when the front tire hit a hole, which caused the steering wheel to jerk and spin around, which caught the right hand, causing the fracture.
The injured was climbing a ladder at the plant and failed to use three point contact and fell approximately five feet to the ground.
Employee was cleaning (shoveling) under the raw coal belt head at the divider to the raw coal shakers. Employee first missed day was 05/09/2006
The operator of 436 haul truck was in the process of dumping on a waste dump berm when his truck went through the berm rolling twice approximately 70 feet in distance before landing on all four tires. The employee was complaining of pain in the right ribs spreading lateraly to the back. Also a contusion to the right eye and minor cuts and scrapes on the upper extremities.
EE was taking engine compartment door off of dozer to gain access to starter. EE squated down to lift door and his right knee started hurting. First day away from work 5/4/06.
Slipped on belt while exiting H-Ride on the south section.
The injured employee was backing haul truck on an ore stockpile to dump load of material for construction of safety berm. Haul truck travelled over face of stockpile and landed upside down on loadout area. Blunt force trauma resulted in two fractured ribs and a fractured vertebra.
BEATING ROCK WITH A 3 LB. HAMMER. RIGHT BICEP IN RT. ARM STARTED HURTING.
Employee was washing the floor, pulling a hose around. He hurt his back in the process.
EE jumped down from conveyor apprx. 3 ft to ground and strained back, he went to his personal chiropractor and continued to work. On 5/10 he was working at VSI and leaned back to put up angle iron and back tightened up, next a.m. he was unable to work and went to see a chiropractor and was restricted from work.
FMC completed review of the surgical report on 6/15 & found that it was recordable. Employees needed to install & re-route compressed air line in order to move the compressors to a new location. Trying to screw pipe into end of other pipe while struggling with weight of pipe (approx. 200 lbs) ee had hold of end of pipe and felt burning pain in lower center of abdomen.
4/27/06 am: 2 Employee changed dragline 3p.m.: Injured complained to the other that his back was sore. 4:28/06:EE reported a sore back-Did not feel he needed to go to the doctor. Said that he must have turned wrong and that this happens every so often due to pre-existing conditions. Degenerative Disc and Austioarthritis. 5/8/06: Reported back sore. 5/9/06:Went to doctor.
Employee was welding, laying down on his chest with out-stretched arms, (out in front) to weld his work. While welding he felt what he thought was stray voltage from the welder in his upper left side. He stopped welding and tried to move, as the pain increased he realized it was not the welder, the pain was in his ribs and growing rapidly. He had muscle strain in that area.
No lost time until 5/1/06. No medical treatment unti 5/3/06. While backing up his bulldozer, the movement of the bulldozer jarred his back. He is receiving chiropractic treatment for the injury.
The employee was swinging a maul and twisted his knee. The employee was beating on a valve that he was re-building.
Employee was walking and stepped on a small rock, causing him to twist his right ankle.
Employee was installing a new leaf spring with an adjustable wrench. The wrench slipped and his hand hit the truck
Employee had made a delivery of material and while driving out of travelway hit a hole, causing him to hit the canopy of the tractor. Lost time reportable as of May 8, 2006.
Employee phoned in 5/1/06 a.m. with eye problem. He indicated that he may have gotten something in it Thurs. (4/27/06) or Fri. (4/28/06) or may have ""pink eye"". He was told to go to a clinic. He was not able to see a Doctor until 5/2/06. Diagnosis - inflamation of eyelid.
Unloading wheel unit of of mantrip, pain in lower back.
Employee was assigned to stack paper bags filled with 50 pounds on pallets 12 rows high. While stacking the top row, the employee lifted a 50-pound bag from a waist-high position, moved to his right and lifted to a head-high position to toss the bag across the stack and to the far corner of the stack. At that time, he felt something `pop? in his right shoulder.
LOCATED ON THE 1ST FLOOR OF THE PREP. PLANT, EMPLOYEE'S LEFT HAND WAS CAUGHT IN THE BELT/PULLEY OF A MOTOR ON AN AIR COMPRESSOR. RECEIVED A TOTAL OF 12 SUTURES IN 4TH & 5TH FINGERS ON HIS LEFT HAND (DISTAL JOINT LACERATIONS).
Employee was working on a conveyor belt located on #2 unit. He was squaring the belt to make a splice using a utility knife. He was pulling the knife toward him while cutting the belt. The knife pulled out of the belt striking him in the right thigh causing a laceration. It took seven sutures to close the wound.
EE was helping move steel beam, which struck him in the left knee.
Injured was walking along #2 main belt conveyor when he stepped on uneven mine floor causing him to fall on left knee. This did not become LTA until 6-1-06.
Employee was hauling rock in a 777D. As he was being loaded, the 992G operator dropped a rock into the truck and it rolled off hitting the side of the truck bed. Jarring the employee inside the truck.
Employee was tracking the drill when it tracked over a large rock. When rock reached the platform where EE was standing, it pushed the platform up against the drill, pinning his foot.
EMPLOYEE WAS DISMOUNTING FROM A D11 DOZER WHEN HE STEPPED ON A CLUMP OF DIRT TWISTING HIS LEFT ANKLE. THIS RESULTED IN A SPRAIN.
Employee went to get out of truck, the door opened to easy and the employee fell out. Employee caught his foot between the steering wheel and the seat hurting his groin area and back.
DID NOT START LOSING TIME UNTIL 5/8/06. While hanging miner cable, felt pull in his groin area. Hernia.
Employee was putting the canopy back on the #2 bridge. He had to put a jack between the canopy & the deck. When he started jacking it up the jack slipped out and caught his hand.
While cutting to headgate, rock came over handrail and hit his left foot.
While kneeling on his left knee the employee was hammering a concrete expansion bolt into a hole when he felt a ""sharp"" pain between his shoulders.
EE was working on conveyor. He slipped & struck his ribs on the conveyor cross brace.
Employee while reaching screen up to change out on #3 screen on 115 pre-wet floor, slipped and fell backward injuring his neck. Although this accident happened on 4/26, employee never started missing work until 5/19/06.
ee was driving a komatsu haul truck, hit a hole in the road and he was holding onto the grap bar and his weight on his left wrist caused a sprain.

EE was energizing the continuous miner breaker at the section power center in the #6 entry of the 001-0/002-0 MMU. When the miner breaker was energized, the cathead blew up, causing flash burns to his hands and face.
The injured and another person were installing conveyor belt structure. The two men lifted a bottom roller and were putting it in position when the injured's left knee buckled.
EE was stepping into the limestone storage building & stepped off a landing and slipped, falling back onto the landing. Reported 4/25; First Treatment 5/4--full release RTW; Returned to MD during work hours 5/8; Missed next scheduled shift 5/12
EE was checking cylinder when he slipped on oil on the floor.
EE was servicing the haul truck. He was climbing the ladder. He hit the inside bone next to the knee cap on the corner edge of the step on the ladder. No lacerations occurred at the time of the accident. The injury point was sore. EE called in accident on 5/3/06, then sought medical treatment, because knee was stiffening up and not getting better.
Employee was in a standing position using a mechanical lifting jack to spread the sides of the miner boom. As he operated the lever on the jack, the jack dislodged and struck him on the right knee.

EMPLOYEE WAS OPERATING A 10-MAN DIESEL RIDE, AS HE WAS BACKING UP HE RAN OVER A ROCK THAT CAUSED HIM TO HIT HIS HEAD AGAINST THE MINE ROOF.
Employee was standing on a conveyor belt working on another belt (2 independent power shut-offs were switched off). Supervisor instructed an employee, in the presence of the entire crew, to start the belts. Injured employee did not think the belt he was standing on would be started but it was. Employee fell 3 or 4 feet and aggravated a previous back injury.
Employee stated that he was installing the inside bolt in the 3rd row of bolts in 6 crosscut left. He had placed the drill steel in the drill pod & raised the drill steel to the mine roof & when the drill steel came in contact with the mine roof, a piece of slate rock measuring 47""L x 22""W x 3"" thick fell striking his left forearm & left wrist causing a laceration to his forearm.
INJURED WAS HELPING HANG HIGH VOLTAGE CABLE. THE CABLE SLIPPED FROM THE HANGERS AS IT WAS BEING RAISED. THE WEIGHT OF THE CABLE PUSHED INJURED RIGHT HAND BACKWARD RESULTING IN A SPRAINED WRIST.

EMPLOYEE WAS HOLDING UMBRELLA TO KEEP WIND OFF OF WHERE MECHANIC WAS WELDING AND HE LOOKED AT THE WELDING FLAME.
A mobile crane was moving a metal shed. An employee was on the ground guiding the shed. The EE on the ground was contacting the metal shed when the crane came into contact with a 13.8 KV powerline. The EE on the ground received an electric shock and fell to the ground. This caused an entrance wound to his hand and exit wound on toe.
Repetitive movement of operating equipment, accumulative trauma over time. No lost time until 7-05-06 when he had surgery.
Employee was laying concrete block and strained right shoulder. Began missing work 5/16/06
A miner was loading a round of explosives at an active mining face (485 level) when hit in upper back by a piece of loose ground. He was bent over, loading the bottom row when the incident occurred. Employee transported to hospital with multiple fractures and punctured/collapsed lung.
Supplying pinner with resin, lifting resin up to put on pinner. Pulled muscle in lower back left side. Came back to work on 4-27-06.
Picking up a broken bag and stepped on bag, spraying lime under his safety glasses into right eye.
Employee was replacing a spring. Spring was being compressed, it slipped out and fell on his right foot.
Employee was walking and twisted his right knee. Employee did not start missing time untill 5/25/06.
EE was walking down the track and stepped in a hole and turned his ankle.
Employee was lifting a smaller toolbox out of a bigger box and felt pain and the right arm went numb.
While unloading 2 inch metal pipe from the scissor lift, 2 pieces started to slip off and employee tried to catch them. The weight of the pipe hyperextended his left wrist.
Employee placed his right hand on the top of the drill pot for balance. He then stooped down to examine the roof area that he was drilling. A rock fell from the roof, approx. 8"" x 15"" x 2"" striking the back of his hand. The employee was taken for x-rays. No broken bone were found. A deep contusion was found between the thumb and index finger.
The scoop that employee was running became hung in a bad hole. The section scoop came down to help him out. EE doesn't exactly know what happened except that he hit his head on the scoop canopy.
Steering wheel on Mac 8 hit left hand while spinning (fractured knuckle).
EE was dragging rock dust hose and tripped. When he fell, he landed on his cap light, bruising his lower back.
Employee was assisting his fellow roof bolter opertaor in hanging the roof bolter cable from the mine roof using insulated tie wire. When he turned his head to move, the excess end of the wire struck his right ear and actually went down into his ear. This resulted in a possible puncture to his ear drum.
Employee walking up spiral staircase with muddy boots. He slipped and fell through the stair rails and onto the ground.

Employee was unwrapping 1.5"" fire hose from a hanger. He was dragging the hose to the tail section of the raw coal belt when the hose caught on an obstacle. Employee pulled on the hose to free it when he felt a burning sensation in his lower right abdomen. Employee did not miss work until 5/8/06.
EE was coming off 4 left overcast when foot got caught in some string. EE tripped and fell, hurt- ing left shoulder when he tried to catch himself.
Employee was coming off the machine down a ladder, his foot slipped off the ladder and he twisted his right knee.

EE's were loading scrap tin to be recycled. The employee moved to lift a heavier piece of scrap to uncover the tin; he pulled a muscle in his back.
Employee is a raw coal dozer operator who was helping the welder this day. He bent over to pick up a chain fall & experienced pain in his lower abdomen. He completed the day & began his 4 days off (works 4 on & 4 off). Saw the Dr. on 4/25/06, had surgery on his left side 5/23/06 & on his rt. side 7/24/06. We learned of LTA 8/15/06 when Supt. called to ask the employee.
Employee was operating a manlift. A loader backed into the manlift at the operator's basket. The employee received broken bones to his left foot.
Employee was working on # 1 Finish Ball Mill using a sledge hammer to clear dust collector pipe when he felt a pain in his lower back, right side.
Employee stepped in a hole outside of shop door straining his knee. ** Employee did not start losing time until 5/22/07.
ee was replacing bottom roller in 6mn takeup. Removed old roller-lifted new roller w/help on other side. Other side went in and stopped. Inj ee's side dropped down, catching finger between roller and mounting bracket.
EE was crushing clay when a piece of grit got in his eye. He was unable to get grit out and his eye became swoole & red. He went to Dr. who said he had scratched his cornea. The Dr. bandaged the eye and advised the EE to take one day off work.
A mechanic was in a kneeling position using an angle grinder to cut through a piece of metal. When finished he stood and when he applied weight to his left leg his knee popped causing pain. A medical exam revealed a possible sprain. He was fitted with a knee support and returned to work. He worked until 5/1/06 when he was scheduled for surgery to repair a medial meniscus tear.
EE was carrying a prop setter jack from #2 entry to #1 entry and stepped in a hole, jarring his lower back.
Employee was struck by a piece of sandstone while walking from controls back to the front of the bolter. He had repositioned the bolter.
Employee was bagging hydrate and stacking the bags, when he felt a sharp pain and burning in his lower back.
Materials Technician was emptying concrete from wheelbarrow when wheelbarrow turned sideways twisting technician's back.
At the start of the shift, the EE was operating track mounted vehicle to his section. As the vehicle went through the 2nd set of airlock doors, an unsecured lid over the traction motor compartment slid out of place and contacted the right support of the airlock door. The lid sprung foward hitting EE and pushed his face into the cover over the contactor compartment of vehicle.
Climbing ladder to do maintenance on screen on our extec screen machine. He said he slipped and grabbed on to the hand rail. He came back to work for next day we requested a doctor visit before returning. He returned on 4/25/06, 4/26/06 and 4/27/06. Then went to a chiropactor and was told to stay off work until futher notice.
Operating clam shell dredge bucket when rock came out and broke the windshield. Glass and the rock hit his right cheek.
EE was riding to the section in the mantrip with the rest of the crew, when the operator went through a dip in the track and applied the brakes. The rail runner skidded into the stop blcok at the end of the track. EE was sitting in the end of the rail runner with his side against the frame. The impact resulted in 2 broken ribs. No one else was injured.
The remote miner was in the #3to#2x-cut and the miner head was penetrated in the A-cut. The operator was standing beside the boom while loading S/C a rock 6ftx4ftx6inthick fell from the top and in the miner conveyor. The rock twisted and fell from the boom before entering the S/C. Luckily only the end of the rock struck operators left leg and calf.
Employee was unloading a portable generator from the bucket of a backhoe loader. As he moved the unit he sustained a strain in the groin area. On 4-27-06 after a doctors visit it was determined that he had a hernia.
EE was on his knee's in front of scoop unloading mega block from scoop bucket. EE twisted around to pick up block from scoop bucket and felt some- thing pop his knee.

EE was operating semi-auto palletizer when he experienced a pop in his right hip resulting in minor pain. He could not continue full duty work so was sent home. EE sought medical treatment the following day, 4/19. EE was moving product bags on air table when insident occured.
EE was adding a chute extension to a readymix truck. The driver pulled forward and the chute ""jumped"" and hit him on the left side of the head. we sent him to the hospital for an evaluatioon to be safe. The hospital Dr gave him the day off to visit his own Dr due to treatment of a prvious injury from an auto accident. He returned to work on 4/20/06. No known problems.
Employe was inserting nip into power center - receptacle for belt drive, felt pain in right groin. Possible hernia.
She was operating a jeep pulling into the fuel station when a motor collided with the vehicle whe was operating causing her to strike her left shoulder and knee which caused a left shoulder impingement. She coninued to perform her regular duties and was taken off work for arthroscopic surgery to repair her left shoulder.
On 4/18/06 employee was loading holes with the prill truck when the boom swung out and employee twisted using his arm to block the boom. He twisted his lower back and felt pain in groin area. Employee RTW 4/19/06. Employee sought medical care on 5/8/06 and now off work due to scheduled hernia surgery on 5/22/06.
Employee was changing out a belt roller, while pulling roller out of position the weight of handling belt dropped down putting the roller in a position which pinched the employee's left middle fingertip between the roller and structure, resulting in stitches.
EE was removing brush from a hillside, lost her footing and slid down the embankment. The fall twisted her right leg and hip.
Repetitive lifting of heavy hammers, crow bars and rock caused a lower back strain (lower lumbar strain)
Changing screens on jaw plant, fingers were in the way when it got moved.
EMPLOYEE STATED HE PICKED UP A BOX OF BOLTS FROM THE SUPPLY HOUSE & PUT THEM ON HIS SHOULDER TO CARRY THEM TO THE CAGE. WHEN HE ATTEMPTED TO LOWER THE BOX THE LOAD SHIFTED CAUSING HIM TO TWIST & CATCH THE BOX. HE SAID HE FELT PAIN IN HIS BACK. MANAGEMENT HAS DEEMED THIS OCCUPATIONAL ILLNESS, IF YOU HAVE ANY QUESTIONS ON THIS ISSUE PLEASE NOTIFY SAFETY.

EE was bolting top when a piece of rock fell from the roof striking the canopy and hitting the EE on the left side of body, causing 2 cracked ribs.
Ee was cutting cable ties and accidently cut his hand

The employee stepped off of a cut in the floor at the face of the mine (approximately 6""), slipped and sprained his back.
EE stated that truck was climbing the hill. The truck stalled and then the truck rolled backward into the berm and turned over on its side.
Raising monorail to hang on chains from the roof. EE rested the monorail on his head.
Running shuttle car toward face, hit hole and strained his back.
While bricking the kiln, bricks fell from overhead breaking employee's right pinky finger.
He was shoveling on beltline, when he went to get a scoop full of coal, the shovel slipped out of his hands, causing him to lunge forward causing lower back pain.
EE was positioning a ventilation tube when it came and struck him on his head, neck, and shoulder off since 4/18/2006.
Employee was attempting to get a broken bit shank out of a bit lug on a miner. He was using a bolt and a hammer, he hit his finger. The cause of this injury was safety awareness. Compliance with regs/rules was a factor, protective equip a factor, miner attitude a factor, mining equip/systems not a factor.
Employee was putting a toggle plate seat in botom jaw. He was lifting about 4 feet high. Strained his back.
Man was changing tire on coal trailer while another mechanic was working on a bar that goes on the automatic tarp cover. The bar sprung back striking man on lower rt. arm. Was taken to E.R. and sent to an orthopedic surgeon to set broken bones.
There was a gob approximately 4"" deep on the offside of the belt drive. EE changed directions while walking and twisted his knee.
Injured was carrying an oxygen tank on his shoulder while going through a man door. It fell and he tried to stop it and it hit the bottom and shoulder at the same time. He said he heard his shoulder pop.
Ee slipped while roof bolting and twisted his knee.
HE WAS WALKING AROUND SIDE OF FEEDER TO TURN IT ON, HE SLIPPED ON SOME COAL AND FELL CATCHING HIMSELF WITH HIS WRIST.
Was running dozer-ripping rock-ripper tooth came loose. Found it in shock and shook it loose with his foot, knelt down on right knee to pick it up and slipped carrying it to the dozer.
Employee was climbing up on Dozier pulling himself with his arm when he felt a sudden pain in his left upper leg.
Employee was putting up roof bolt in the face of the #3 right cross cut when the boom came down unexpectedly and struck him on his left knee. Possible torn ligements to the left knee.
Ee was installing a chute on a return belt. He was using a pry bar to align the bolt holes. He turned to speak with another ee and felt pain in his chest. He reported the incident and was directed to seek medical attention.
EE was using a pry bar to remove shims from the miner. The bar slipped and EE fell & hit the mine with his shoulder.
Traveling through mandoor from the return to the intake and his leg became caught in the door knocking him off balance. He fell hyper-extending his right knee causing severe pain to radiate from his knee. He began losing time on 4/17/2006.
Injured was using a 16# sledge hammer to break rock at a rock fall. injured was swinging sledge hammer and felt pain in his arm. (Injured did not request medical treatment until 04/28/06)
EE stated he was cutting a piece of metal at the metal rack. While working, he stepped on a piece of angle, which turned or rolled and this caused him to twist his knee. EE finished his shift.
EMPLOYEE STATED WHILE BOLTING 3XL CROSSCUT, ON 4/15/06, A PIECE OF SLATE ROCK FELL STRIKING EE ON THE RIGHT SHOULDER. EE WORKED ON 4/17/06, AND TURNED IN FLOATING VACATION DATYS FOR 4/18 & 4/19. BASED ON MEDICAL INFORMATION RECEIVED ON 5/25/06 IT HAS BEEN DETERMINED THAT EE SUFFERED A OCCUPATIONAL INJURY.
WHILE WORKING ON TOP OF SAND PLANT, EMPLOYEE WAS SHOVELING ROCK TO UNCLOG THE SCREEN, A ROCK THE SIZE OF 2 SOFTBALLS BOUNCED FROM THE SCREEN & HIT THE LEFT SIDE OF EE'S HELMET.
Employee stated he was bending over picking up dust bags that the pin crew had changed out that were full of dust. While throwing the bags into the scoop bucket he twisted and felt a pain in his lower back.
Gas leak in Revolving Frame was ignited, causing flash fire.
Employee climbed onto stationary equipment & shoveled rock from equipment decks throughout the day. He did not seek medical treatment until 9/7/06 at which time it was determined he has a hernia. Last working day was Wed. 9/6/06. Is taking antibiotics for a related infection, then will schedule appointment with surgeon.
EMPLOYEE WAS WELDING IN SHOVEL AND DRILL SHOP WHEN HE TRIPPED OVER PART THAT WAS BEHIND HIM.
He was climbing up on end of loader and he felt his little finger hurting on his left hand.
EE states he was working on top of silo with 3 others and there was grinding and welding in a small area. EE was taking up grating, when he was hit in the right eye with a foreign object.
Per the treating physician, metal shavings with some rust entered his left eye causing discomfort, blurred vision, & an infection the following week. It is unclear exactly when & how the injury took place. It is believed to be during a job on April 14 involving a cutting torch & grinder to remove nuts & bolts from a whizzer blade. Appropriate eye protection was being worn.
EE was lifting a battery out of a Jumbo drill w/a lifting strap. As he got about chest high, the right clamp slipped off and came up & struck him in the right eye, causing trauma to the eye w/ damaged blood vessels.
Was building concrete block wall over hoist drive cabinet. Dropped conduit through cable tray into circuit breaker cabinet landing across 2 phases of breaker causing an electrical arc which burned him.
EE was using a cutting torch and cutting the bolts out of a cyclone in prep plant cyclone was suspended by a chain hoist. When EE hit cyclone with hammerto knock bolts out of the cyclone it broke free & swung around striking EE on left forearm.
Hanging 8"" blue mine pipe, slipped and fell & hit right shoulder. **EMPLOYEE DID NOT LOSE TIME UNTIL 10/28/06 AND RETURNED TO WORK 10/30/06** **Employee had surgery 1/8/07 & is losing time again**
Employee was decending the ladder on a haul truck. When he turned on the landing to complete his decent, he twisted his right ankle resulting in a sprained right ankle.
While shoveling on belt, employee possibly pulled a muscle in back.
EE slipped and fell at the tailgate while carrying an oxygen bottle.
Employee was removing belt structure when his index finger on his left hand got caught between two pieces of belt structure. Employee sustained a fracture/laceration to the distal joint of the index finger.
The employee stepped from the ground onto the lower end of a conveyor belt (approximately 18"" above ground) and slipped and fell onto the rubber conveyor belt.
EE was cleaning a scale basket in area 325. He was not wearing goggles and caustic scale flew up into his eye.
While roof bolting, employee was removing the drill steel from the bore hole when the drill steel struck him below the right eye (cheekbone area) causing a laceration.
While installing roof bolts employee was holding his first steel in the hole with his hand and while trying to insert the second steel into the first steel employee accidently raised the boom and caught his index finger on his right hand between the second steel and the mine roof causing an evulsion. Employee received 8 sutures and a skin graft to repair evulsion.
EMPLOYEE STATED THE INJURY OCCURRED AS THE OFF HIGHWAY TRUCK HE WAS OPERATING WAS BEING LOADED BY A FRONT END LOADER. EE DID NOT MENTION THE INJURY OR DISCOMFORT AT THE TIME HE SAYS THE INJURY OCCURRED.
Employee was working on dozer and when he stepped off of the platform by the cab, his feet slipped off of the track. He fell on the push-arm and floor. He fractured his left leg and was taken off of work.
IN AN ATTEMPT TO PREVENT AN 1,100 LB. BAG OF KAOLIN FROM FALLING OFF TOP OF STACK, EMPLOYEE TWISTED HIS KNEE AS HE MADE AN EVASIVE MOVE TO KEEP BAG FROM FALLING ON TOP OF HIM AS HE FELL TO FLOOR.
The employee was carrying a bucket of gear oil and an impact wrench when he lost his footing in mud and felt a pain in his groin. He worked until 6/1/06 when he was diagnosed with a hernia and has surgery scheduled for 6/15/06.
EE alleged while he was picking up cable & placing them on back of pickup truck he hurt his lower back.
EMPLOYEE STATES HE REACHED TO MOVE CABLE WITH HIS FOOT AND HIS FOOT BECAME TANGLED IN CABLE LOOP, MINER HEAD STRUCK HIM IN LEG.
Employee was descending a set of 3 steps. He tripped and fell into the handrails of another set of steps, breaking 3 ribs. Started losing time 4-14-06.
Employee was jack hammering at #2 drag while 2 other employees were vacuuming with vac hose. The vac hose became plugged. When vac operator unplugged vac hose, the hose jumped, latched on employee's leg. He pulled the hose off his leg and it grabbed his arm.
Employee was passing thru curtain and bumped his head on scoop. ***NOTE***Employee did not seek medical attention until 05-12-2006. Started missing work 05-13-2006
EE removed guard and was cleaning off area. Motion sensor arm came around and trapped EE's finger between arm $ sensor resulting in injury.
Water truck would not stop going in reverse & rolled over edge of cliff into ravine EE jumped out of vehicle on to the groung dropping about 6-8 feet to avoid serious injury. EE broke L heel sprained knee, R shoulder & side received cuts & scrapes from foliage.
Two employees were hanging miner cable when the cable was pulled down by the machine operator pulling up extra cable.
Miner was assisting in moving a scale house. A beam fell out of the loader bucket and bruised miner's lower left leg.
Hydraulic line broke on Hitachi 1800 Shovel, employee slipped on oil and fell.
Stepping from last step of a stairway onto a concrete floor he felt a sharp pain in his right leg. EE neither slipped or fell.
On 4-17-06, the man reported that on 4-12-06 he felt a pain in his lower back while lifting belt structure.
EE was walking down supply entry #2 to check to see how many timbers was down there when a piece of slate rock 1 ft.x 1 ft. 1/2 in thick fell out & struck him between shoulder blades injury happened at spad ""3400""
While roof bolting a piece of rock fell from the roof striking the employee on the side of his head causing brush burns, a loose tooth and tingling in his right arm. The tooth was pulled by a dentist.
Cut, puncture to right elbow. Fell hit elbow on stone, causing puncture and cut to right elbow.
EE struck left leg against steel saftey step at #4 kiln 2nd piece lacerating leg. Due to injured leg vascular disease, the wound became infected requiring RX's upon Dr. visit on 4/27/06. Addendum: This injured leg developed minor clotting requiring disability (lost time). Lost time started 5/3/06.
Assisting with the installation of a rod on a water truck, strained his back
Injured was removing scrap from R3803, which was shut down. A kick out gate was opened, Employee placed hand in gate to remove scrap & gate closed on his right hand. Resulted in a fracture & lacaration to right hand fingers.
Loader bucket slipped off a rock and the jar from the loader caused pain in his lower part of his back. He also said he had some pain the day before.
NOTE: Unaware this had to be reported, as accident occurred in parking lot outside normal work activities and shift. Emp was parking motorcycle in company parking lot when the motorcycle fell on her, causing fracture to right knee which required surgical repair.
EE LIFTED A STEEL SHAFT FROM A CONVEYOR LATER IN THE DAY WHEN HE WENT TO THE BATHROOM, HE NOTICED A LUMP IN HIS GROIN AREA AND REPORTED IT TO SUPERVISOR.
EE was cutting, a metal beam with a torch when a piece of hot metal went down his shirt. This resulted in burns to his chest.
Employee was trying to release a rock that was caught in a tailgate of a dump truck; as he pulled on the tailgate the rock released catching his left hand index finger between the tailgate and the rock. The rock smashed the tip of his finger causing a fracture. Employee did not use a pry bar to open the tailgate as instructed in previous training.
While working at the kiln, lime dust blew into employee's right eye.
Employee alleges he was backing up dump truck and he flipped it over on its side.
EE was repairing the ladder on the dredge over solid ground when EE stepped off ladder onto a large dirt clod causing him to twist and sprain his left ankle.
EMPLOYEE STATED HE WAS HELPING MOVE THE CONTINUOUS MINER CABLE OUT OF THE HAULWAY, WHEN HE TRIPPED OVER A RIB ROLL & FELL STRIKING HIS LEFT LEG JUST BELOW THE KNEE ON A LUMP OF COAL ON THE MINE FLOOR. EMPLOYEE WENT TO THE E.R. AFTER WORK TO HAVE HIS LEG CHECKED & RETURNED TO WORK ON HIS NEXT SHIFT. EE CONTINUED TO WORK UNTIL 4/22/06 WHEN HE RETURNED TO THE DR.
Employee was firebossing the rotary dump. He was between levels 4 and 5. During his last step he slipped and grabbed the handrail to prevent a fall. He stated that he felt pain in his right knee. The steps were wet and muddy. Employee began missing work May 3, 2006.
While stepping on copper starter sheet, his left foot slipped causing his left knee to twist.
The injured EE was struck above left eyebrow by a foreign object, while lugging steel ties to track rail. An attempt to remove object was made by physician. EE had 7 stitches to close wound made by the physician.
EMPLOYEE WAS WALKING TO THE OFF ROAD TRUCK HE WAS GOING TO OPERATE WHEN AN INSIDE TIRE OFF A PARKED TRUCK BLEW-UP CAUSING A LOUD NOISE.
Moving miner back and went to move cable and felt pain in upper back.
Employee was using hand chisel to chip stone in the quarry. When employee struck chisel with hammer, a metal sliver left chisel and entered left lower leg.
Employee felt pain in his lower back as he was loading supplies. He continued to work through 04/27/06 at which time the Doctor removed him from work.
Employee stepped on a pallet to pick up fittings and the pallet gave way causing him to fall onto left side. Employee did not start losing time until 9/7/06 due to surgery.
While scooping #7 entry employee reached for a piece of curtain to throw it out of the way when his knee hit the steering lever and the scoop swiveled pinning his left wrist against the rib. Employee suffered two broken bones in his left wrist. This accident became lost time as of 4/11/2006.
EE was helping mechanic splice his car cable. He had layed the cable across his knee to cut outer jacket and accidently cut his right knee.
EE was located on right side of roof bolting machine. He was standing 3 rows of roof bolts back from last row & was watching the mine roof for his off-side operator while he installed bolts in area of adverse roof conditions. As he observed the roof, a piece of rock measuring 4'x4'x2"" thick fell without warning striking him on head & shoulder & landed on R foot.
EE was using a bottom belt roller to pound a christmas tree water valve on the end of the east water line. While pounding his thumb was pinched between belt roller and the fire valve. x-ray showed fracture to his thumb. EE did not return to work his next scheduled shift.
Employee was roof bolting in the face of #7 entry on #1 unit when his left knee slid on a small piece of rock, that was on the ground, causing his knee to twist. This did not become a Lost Time Accident until July 3, 2006.
Person was carrying bag of rock dust on shoulder to use for dusting on beltline. When he walked past starter box he stepped on rubber mat and slipped and fell causing injury to left knee. Person now has to have surgery on knee, first day off is 9/21/06.
Stacking frames and they came over on him and knocked him off the truck. (Broke both legs).
Employee was working at the semi-automatic palletizer table positioning bags onto pallets when his right arm began to hurt. He switched positions and operated the forklift for 1-1/2 hours, then went back to the palletizer but his arm was still sore. He went home at 8:00 pm in the middle of his shift. Employee sought medical treatment the next day, April 11, 2006.
EE stated that he was using his left hand to steady a block that he was going to cut for a footer. When he swung the axe with his right hand the axe handle hit his belt and spun around, causing the head of the axe to hit his left hand.
EE was shoveling behind line curtain on the 9 right section in # 4 entry and felt pain in lower back. EE finished working remainder of shift. EE started missing work on 4/14/2006.
Employee was putting a relay bar pin back in that connects to the knuckle when the shearer caused the pan to move, pinching the employee's finger.
Employee was servicing a haul truck. When he lifted a 5 gallon bucket of oil over his head, he heard and felt a ripping in his shoulder.
EE is a belt maintenance man. As he was watching belt run, he saw a roofbolt laying on the belt. He reached to pull bolt off the belt and his right middle finger got caught in between the moving belt and roller.
hard packed icy surface led to employee slipping on the ice and injuring his wrist.
Employee lost footing on stairs and slipped. ***EMPLOYER WAS NEVER NOTIFIED OF CLAIM UNTIL 2-20-2007
While inspecting the skid loader after operating, the EE resulted with minor burns on hand and chest when he removed the radiator cap.
Employee swung the bolter head and caught his right knee against the rib. Double head bolter.
OPERATING BUGGIE WHEN HE HIT A HOLE IN THE ROAD & WAS THROWN UP AGAINST THE CANOPY.
While loading supplies, employee stated he felt pain in his lower back. He didn't start missing work until 8/16/2006.
Employee was bolting top when a rock measuring 2""x4""x22.5"" fell & struck him on his foot(L).
EE reported when moving the continuous miner cable from the ram car haul road, lost his footing and stumbled. When he caught himself to prevent falling to the mine floor, pain was experienced to his low back. The pain and discomfort continued and on 4/14/2006 sought medical treatment. Continued to work until 4/16/06 started missing.
Employee pinched her arm between the arm rest and truck door. She didn't wish to seek medical attention at time of accident. It continued to bother her and in July was treated and released to full duty pending surgery date. On 9/12/06 employee had surgery to correct neuropathy and was taken off of work. It became reportable at this time.
19 year employee, scooptram operator, alleges banging head on canopy numerous times throughout career resulting in cumulatiuve trauma to the neck.
EE was bolting top. He had put up outside bolt and swung head in. Rock broke out between personal canopy and ATRS, EE started to run. Rock hit EE's right leg 6"" above his ankle, breaking his leg.
INJURED WAS HELPING TO TAKE CHAIN OFF A SHIELD CARRIER. HIS FOOT SLIPPED OFF THE CARRIER CAUSING HIM TO TWIST HIS RIGHT ANKLE.
EE was trouble shooting the gathering head circuit on JCM-4R in panel SA02. He opened the main control box on the miner and was checking connectors, when he reached in the box, he touched lugs and did not check power or lock it out and hit two phases of power.
Employee was standing behind the roof bolter and was knocked off his feet when the other operator took up the cable. Management questions the validity of this accident. Employee notified management on 4/13/2006, that the reason he'd missed work was due to this accident.
EE was lifting a tail lift cylinder, when it slipped off his leg and caught his finger against metal.
Injured employee was working on a belt splice when he stepped on a rock or lump of coal and twisted his right ankle resulting in a broken bone just above the ankle.
The EE stated while lifting rub rail on miner, he felt something pull in his back.
Employee was hanging miner cable when he strained his right shoulder.
Ee was lifting a heavy object. It slipped and caused him to fall.
Process of rolling a tank to reposition. EE was told to move his chock since it was loose. There was a manway in center of the tank so had to put the forklift on the other end of the tank to releive pressue from bound end to remove that chock. At the same time the bound end had been freed the forklift was backing off tank to reposition. Tank rolled & pinned EE R Leg.
Laying interlocking block for seals & working on knees. Spilled concrete mix that got in employee's clothing and caused chemical burning on knees and shins.
Employee was reporting for work. After checking with the train operators, he attempted to climb over two coupled train cars when he slipped and fell, injuring his left hip and elbow.
EE was repairing a broken conveyor chain and pulled his glasses off because they were fogged up. He turned to speak with co-worker, but when he turned, co-worker struck a sprocket chain gear with his hammer. Something hit EE's eye. EE went to an eye Dr. on 4/7/2006 and metal was found in his eye.
EE caught right foot when crossing jack pipe and tailpiece in zim section. Torn MCL/ACL Off since 4/14/2006
Injured was rolling rail with slate bar when it rolled and hit the other rail and rolled back onto his right foot and bruised it.
Closing lid on pneumatic trailer using a 8' pole from catwalk above loadout chute. Leaned over the handrail to reach lid, it was stuck requiring extra force. He felt a slight pull in lower back but continued to work. Reported injury on 4/07/06 but did not seek medical attention until 4/11/06.
The operator climbed up a horizontal conveyor belt to clean it off after it plugged. Due to rainy condition the frame was slick causing him to slip.
EE was getting onto dozer and twisted his back. He went to the Dr. on 4/8/2006. Dr. believes it to be a pulled muscle. EE came back to work on 4/7/2006, but is returning to Dr. for reoccuring pain.
Finished working on haul truck 274, doing paper work in cab of service truck. 274 honked horn pulled foward turned right and ran over service truck.
A flop gate (75 lbs) at the end of a conveyor had rolled up and landed on the tail pulley. Two employees raised it up to let up back in position. One employee let go of his end after the gate went over center before the other and the gate pinched the right ring finger of the second employee against the angle iron that the gate swung in.

Employee used disconnect switch to open pringle switches and isolate capacitor. One switch hung up and did not open. Employee attached jumper to ground, then to capacitor to drain off power stored in capacitor. Since the system was still energized, a massive power surge occurred. Fireball caused injuries.
Dismounting off drill and got to the last step and lost her footing and fell to the ground.
8I section, using loader to pull up slack cable. He walked back to move cable out of tram road. Rock fell from rib and roof line, striking his left foot, causing fracture to his big toe.
EE tripped over material. Fell and injured right ribs and right knee.
EE was struck by a pipe which had been stored on top of a large tires to keep pipe out of the snow. It rolled off the tires while the injured was standing near the tires. He has just lifted a a small boat off tires and while retrieving the boat oars the pipe rolled and fell on his L ankel fracturing it pipes removed and placed in pipe rack.
OPERATING DOZER OVER A MOUND OF DIRT CAUSING THE RIGHT ELBOW TO HIT FIRE EXTINGUISHER. CUT ELBOW AND CAUSING SWELLING OF R. ELBOW.
On 4-5-06 employee was tramming the bolt machine from the #4 entry to the #5 entry. He backed the bolter out of #4 and was turning toward #5. While pivoting the machine with the inch controls, a fly board lying in the fly board rack went against the rib and swung out toward ee hitting him in the left temple area.

EMPLOYEE WAS DUMPING A BAG OF FLOOR DRY INTO THE CONTAINER WHEN HE HAD A SHARP PAIN IN HIS LOWER BACK.
EE was swinging lare componet (777A Brake unit) with service truck boom. He was watching the top of the boom to check clearance while swinging boom. As a result he was trapped between compartment and service truck.
EE was bolting top in #6 heading on #2 section, drill steel fell out of hole and landed on his left middle finger and ring finger.
Employee did not have eye protection on and suffered welder's flash.
Employee injured his lower back while installing troughing rollers on F2 conveyor.
Operator was leaving the North Stacker pad to end night shift. The operator decided to go down along the 13B. Traveling down from the head the operator was unable to make the left hand turn in a downhill descent. While making the left hand turn the operator got the CC-113 pick-up parallel with the berm and rolled the vehicle down the embankment.
A truck was returning to maint. shop from the mill heaping east on south side of maint. shop. EE was operating a forklift hauling west. The truck was on the S. side of the hydrant and did not see the forklift. Both were still moving when they hit. Trying to reach Dr. for claificatio of discrepancy between written report and release he gave EE. D&A performed on 4/6/06 were neg.
Emp was shoveling all day and is complaining of lower back pain.
THE OPERATOR WAS REPAIRING THE BAGHOUSE SCREW CONVEYOR. THE WRENCH SLIPPED, STRIKING HIM ABOVE THE LEFT EYE.
Employee removed exhaust hose from water pump prior to moving pump. When exhaust hose was removed, the hot water from the reservoir covered employee's legs and left arm.
Drilling, standing in overburden (mud), tried to move, right foot became struck, heard a pop in leg/hip.
Inj employee was working with an electrician they were having trouble shutting the power off a particular electrical service, so inj ee continued to try and disconnect the power and in that process there was an arch of electricity that caught ee's left arm and jacket on fire, he received a serious flash burn to the face and his left arm.
Loading in shot, large rock rolled off the top of shot and hit on loader bucket. Dislocated 4th and 5th vertebrae in operator's back.
EE stated that he was helping another employee unload props from the top of Petito Mule. EE dropped his end of the prop causing it to fall to the mine floor. injured said it jarred his shoulder causing pain.
Unknown; EE did not report the injury to anyone, we learned of it on 4/5/06. EE's daughter states that he dropped a piece of slate stock he was wieghing, on his finger.
EE was loading metal plates to repair a chute onto his welding truck when one of the metal plates fell onto his left pinky finger causing a deep laceration.
Injured employee received smoke inhalation from the incident when another employee caught fire from an electrical arch.
Running shuttle car to miner. On the way back to feeder, injured hit hole in mine floor. Hitting this hole caused him to jarr his lower back.
Installing 6"" yellow mine water line. Put 2"" shut off valve on end. Filled line with mine water. Opening 2"" water shut off valve to purge line of rocks and debris. First coupler outby failed and jetted 6"" line inby. EE's hand was on valve causing hard jerk on shoulder.
Employee was operating his haul truck and parked at muck pile. A 114 inch rock rolled down face of the muck pile striking the back of the truck jolting the driver and causing his back to tighten up.
Employee was entering the bathroom stall and when he closed the door it fell off the hinge and landed on his right foot. He was evaluated by a physician and he was diagnosed with a foot fracture and his foot was immobilized.
EE was pulling with both arms on curtain, when he felt something pull in his right shoulder, causing pain.
Employee smashed his finger while helping to install bolts in the bradley mill.
Unit #2 - Employee was operating miner, rerouting back across the section. After backing out, the cable was in the roadway. Employee picked up cable and walked across to the rib line. Right lower leg muscle gave out.
Uneven ground around conveyor which repairs were being done on. Employee stepped into a shallow hole. Resulting in back pain. Later diagnosed as herniated disc.
Climbing onto Service Truck bumper striking head on truck crane causing pain to the neck.
EMPLOYEE WAS WALKING FROM #3 ENTRY TO #2 ENTRY. AS HE TURNED THE CORNER TO GO TOWARD THE FACE, HIS HARD HAT CAME IN CONTACT WITH A BELT HANGER USED TO ANCHOR SHUTTLE CARS, CAUSING PAIN IN HIS NECK.
TOP WAS LOW AND ROLLING, EMPLOYEE WAS TRAMMING MANTRIP GOT HIS HEAD CAUGHT IN THE TOP AGAINST THE MANTRIP, TWISTED NECK
While stapling the air hose got pulled which made the stapler jerk and stapled thumb.
Employee was hanging miner cable, picked the cable up and felt a pain in his back. The roof was 5 1/2 to 6 feet high and he was lifting cable by himself.
Employee slipped on rock and twisted his right knee and fell on his left shoulder. Lost time injury as of 04/29/06.
EE was lifting bundle of bolts abdominal pain diagnosed double hernia surgery scheduled for 4/20/06 off since 4/17/06.
Employee was carryng a can of hydraulic oil and strained back. He started missing work on 4/13/2006.
EE had performed repairs on screw drive end assembly. When equipment started up, it locked down causing bolts to break that secured the drive assembly. The assembly rolled striking EE and knocking him to the floor.
EE was welding while standing on a ledge approx. 4-5' above the ground. EE slipped on loose material and fell down, breaking his ankle.
EE was hanging curtian in cross cut for scoop to get thru to claen. Scoop car backed over foot.
Ee was removing a rock from the top of the miner and hurt his back
Employee injured his finger (R-Index) when the mantrip that he was riding was struck in the back by another mantrip. (Finger smashed - struck by batteries).
Injured was standing on cement block to plaster when the block shifted and he fell and twisted his left ankle.
EE was walking along the rear of a fletcher roof bolter and was unable to lift his leg was due to preexisting arthritic condition. EE was seen at local hospital & ER. Dr advised EE there was evidence of arthritis in the back of knee area. He is following up with his family DR.
EE was on mantrip coming outside after completing shift. A 15-ton motor collided with the mantrip. EE was taken to the Williamson Memorial Hospital for treatment.
EMPLOYEE STATED HE TRAMMED THRU FLYS STRIKING OTHER HAULER. EE STATED THAT THE OTHER HAULER DID NOT HAVE LIGHTS ON. ALSO STATED OTHER HAULER WAS IN A DIP & HE DID NOT SEE THE HAULER.
EE was operating dozer on evening shift. It was a cool night so he decided to turn on the heat. EE backed dozer off pile, exited dozer, lifted engine compartment reached inside and shut off one valve then as he reached for 2nd valve he struck his right hand into the belts& pulley system causing multiple fractures 2 severed tendons & 1 severed artery on his index finger.
Burst disk on truck 639 blew and sprayed emulsion from head to toe and got into his eyes.
Employee was in annual re-training, going through the smoke truck. When he came down the stairs of the truck, his ankle rolled on the pavement.
While lining up ties, employee hit the top of his foot with a six foot long bar.
EMPLOYEE REPORTED: OPERATING ROOF BOLTER IN FACE AREA, INSTALLING RIB LINE BOLT. WHEN STARTING THE DRILL HOLE, A PIECE OF DRAW ROCK FELL, STRIKING THE ROOF OF THE CANOPY. A SMALL SECTION OF THE LARGER ROCK BROKE OFF (2' X 2' X 2-3"") AND FELL, STRIKING HIS MID RIGHT CALF, RESULTING IN A SEVERE LACERATION TO THE CALF.
Area is muddy due to significant rain. Employee was cleaning under the pit belt feeder by shoveling out the material. His foot slipped in the mud while shoveling and he twisted his back. Employee transported to Doctor who indicated that he was unable to return to work.

employee was replacing product screen at the kiln when he bumped a beam, while lifting the screen to another employee up above causing lime to fall on his face and in his eyes.
EE was standing on End Loader bucket checking connection pins. EE slipped and fell, landing on his back, resulting in a sprain/strain.
EE was checking fluid levels on a O&K hydralic shovel, when he slipped and fell against his left wrist, spraining it.
EE was using a torch to cut metal for a coal turn chute when he twisted coming off the belt and fell.
Trk 112, retorque left rear inside wheel using RAD 30 torque tool. Holding RAD-30 with right hand @ trigger, left hand @ tube of tool. Caught left thumb between tube and wheel - reversed tool to pull hand out. Reported to tech, called Security, transported to hospital.
Employee was in the prep. plant shop and was dismounting a 553 Case Bobcat. He was doing repair on this piece of equipment, hung his toe of his boot and fell to the concrete floor. He tried to catch himself and he landed on his hand and shoulder. (Left). Employee has worked every day but underwent shoulder surgery 06/26/2006.
Employee was driving a t-post with a t-post driver, as he raised the driver up above the t-post the driver had come off the top of t-post, on the way down, the employee's finger was caught between the t-post and the handle of the t-post driver. This caused the end of his finger to be severely cut between the two pinch points.
Employee was laying cat-head and cable over in the HWM car when the bottom of the pusher jack caught his left foot, resulting in 11 stitches.

Employee was stepping off ladder onto a concrete deck. The concrete floor had just been washed down and when he stepped onto the concrete his foot slipped causing his right knee to twist. Employee did not seek medical attention at the time, but later decided to seek medical attention and had surgical repair on 5/25/06. He is currently off work.
Employee left manbus in gear. Injured employee walked between manbus and shuttle car. Another employee accidently hit tram pedal causing manbus to move forward catching injured employee between manbus and shuttle car.
The millwright was kneeling on a conveyor belt that was laying on the idler deck, as he repaired the belt. The idler deck was elevated 36 inches above the concrete floor. As the millwright tried to climb off the belt, he caught his foot and jacket on an idler. He then lost his balance which caused him to fall off the conveyor and land on the floor.
As Employee was going from the volvo truck to the excavator he slipped and landed on his left arm & elbow causing scrappers and cuts.
EMPLOYEE WAS INFORMED BY CONTRL RM OF A PLUGGED CHUTE ALARM. EMPLOYEE WAS REMOVING CHUTE COVER. WATER CONTACTED DRY LIME CAUSING A BURP THAT COVERED HIM WITH LIME ON FACE AND BODY.
Employee slipped while exiting the dragline in an improper manor; instead of exiting at one of the four proper exits, EE made a shortcut to what is called the shop (not the correct way to exit). The result was that he slipped & landed 3'to the ground & broke his wrist.
Employee's hand became pinched between a manlift basket and a crusher flywheel.
EE was driving haul truck approaching dump hopper. Operator in a 988 CAT Loader backed around surge pile and backed into front side of Euclid.
Injured employee was standing on 3rd rung of a portable ladder when it slipped out from under him. A vertebra was fractured when he fell to the work platform.

Employee was chaining a quarry block, the chains were tightened when lifting and his fingers were caught between the chain and the block.
Employee was operating a CAT 980H shipping loader when at approximately 7:40 pm he asked to talk to his foreman. The employee climbed down the ladder and when he stepped onto the ground he felt a burning sensation in his foot. The employee was diagnosed with a foot strain on 4/27/06, and at his reexam was placed on restrictions on 5/2/06.
Climbed on lift truck, lifting up with left leg, he felt sharp pain and stiffness. Swollen at end of shift and during shift.
Stepped on a rock roll, twisted knee.
EE was attempting to remove hammer hose from a hydraulic hammer. Though the pressure had been released but as he was removing hose blew off and struck saftey glasses. Minor cut under eye but further exam revealed fracture on bone.
Employee was in a hurry and walked down a ladder the wrong way (forward) when his foot slipped off the step. He fell approximately four feet onto the ground injuring his right arm and lower back. He scraped his arm and has a sore lower back.
Employee attempted to climb back onto his machine when his foot slipped off the cat walk causing his right lower leg to strike corner of cat walk. When his leg struck corner of cat walk it caused a 14cm laceration to the right lower leg.
Employee was cleaning from under the screen trailer with a shovel. He stepped down on a rock and bent his ankle to the left. It didn't start hurting until the next day.
The injured and a co-worker were cleaning a hopper when a chunk became loose and struck the injured on the right leg.
