Accident type, without injuries

A roof fall occurred on the Main North Track at 23 X-cut, 26+20', #5 Entry. Fall was 30' long X 30' wide X 7' high. Area was mined in Oct of 88'. Fall occurred due to the immediate roof deteriorating around existing roof bolts. Area was supported with 8' and 10' expander shell roof bolts with a 2' resin anchor. Planks, header blocks and cribs were also used. Dangered off.

A roof fall was discovered in the 2 west #1 mains, 21 crosscut, #2 entry. The fall measures approximately 20' long, 20' wide and 8' thick. The area is supported with 6' point anchor bolts.

Main Portal Elevator stuck on bottom landing two times during morning and doors would not open. Adjusted outer door lock switch and replaced pulley and broken cable on outer doors on elevator bottom. Elevator put back in service at 10:17 a.m.

Upon examination of the left return a roof rall was discovered. The fall is located at spad 401 on the 2nd south west mains in the left return. The fall is approximately 20 feet wide x 9 feet high x 90 feet long. The fall did not affect miners, ventilation nor equipment. The area has been cribbed and dangered off.

An unplanned roof fall occurred in the 8 Left Rooms area, 1 crosscut inby Station number 4642. This was discovered during a weekly exam of this area. The fall was approximately 6 feet high, 19 feet wide, and 70 feet long. Normal ventilation was not disturbed. The affected area was timbered and dangered off.

A roof fall was discovered in the 2 main west #1, 29 crosscut, #4 entry. The fall measures approximately 30' long, 20' wide, and 9' thick. The area is supported with 6 foot point anchor bolts.

The Cedar Creek elevator was out of service from 8:00 am on 1/31/07 to 2/1/07 at 8:35 am due to a problem with the doors not functioning properly. From the Kemper was on site and adjusted the doors and instabled/a new operator which corrected the problem.

In the #9 entry 1 blk inby spad #5148, a roof fall 8'h x 25'lx17'w occured. The area was mined 4 years ago, it is not a normal travelway. The mine examiner noted a chg there week prior to the fall and set breaker posts & dangered it off expecting a fall soon. The area was bolted with 5/8 x60"" resin bolts with 8x8 plates.

Fall 40' x 20' x 4' occur in the main intake escapeway 14 breaks from fan and 11,000 feet from face. Withdrew men from mine and reported fall to agencies.

A linkage on the hydraulic brake on #2 hoist (personnel and material hoist) broke causing the brake to set. The hoist was down from 2:56 am until 11:45 am (8 hrs, 49 min). A new linkage was installed.

Rock fall measuring 25' x 16' x 3' fell on #10 belt 2 breaks inby at Horse Creek punch out. No injuries occurred. Fall was cleaned up and supported per roof control plan.

On 1/29/07 an unintentional roof fall occurred on the 2nd West Sub-Main off the 4th North Sub-Main in #5 entry at cross cut #20. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 16' long x 18' wide x 14' high. The roof fall will be cleaned up.

The east Finlay elevator was down for more than 30 minutes due to the brakes needing adjusted. There was no entrapment.

A roof fall was found by pre-shift ecaminer on 1/28/07 cross cut 68-69 entry 6, 6IMS. Fall was in 4-way intersection that had been mined December 2005 and supported with 5 and 6 foot resin bolts. No injury or property damage resulted. There are no plans to clean up fall.

The cedar creek elevator was not of service from 10:05 pm 01/28/07 until 5:15 am 1/29/07 due to water in the bottom of the shaft.

A roof fall was found by pre-shift examiner on 1/28/07 at cross-cut 132 entry 8 main south. Fall was in 4-way intersection that had been mined July of 1992 and supported with 8' full resin bolts. No injury or property damaged resulted. Fall will not be cleaned up.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN X-C 37, #2 ENTRY IN THE SECOND WEST MAINS. THE FALL WAS APPROX. 30'W x 30'L X 9'H. THE FALL OCCURRED IN THE PRIMARY ESCAPEWAY. THE AREA WAS DANGERED OFF, ADDITIONAL SUPPORT INSTALLED AND ESCAPEWAY RE-ROUTED.

The Hart street elevator failed to operate when the door switch located on the outside of the cage bacame wet due to a leak in the shaft lining. The crews underground were routed to the South portal and removed from the mine. The switch was replaced and the elevator was returned to service.

Roof fall 30 x-cut of 1 mains between 0 and 1 x-cut of 2 mains. No injuries, no impairment or damage.

Continuous miner was pulling last rib on block when a roof fall occurred causing continuous miner to be trapped under rock fall.

At approx 12:30 p.m. smoke was detected from duct out of EEF8, the fan for EBH8. The diverter gate to #1 Separator was closed off allowing all the heat into EBH8, resulting in ignition of bags. Odessa Fire Dept responded and no action was taken. Fire destroyed about 960 bags.

An unintentional fall of roof was discovered during a weekly check of the Right Fork Submains. The fall was located in the 2nd left panel, break #32-33. The fall was app. 20' x 90' x 7' in the #3 entry.

At 5:56 pm the Miracle Run elevator stopped in the shaft @ approx. 50' down from the surface landing. The elevator had 20 day shift employees on board. Elevator was restored to operation at 7:15 and the employees were removed. It was determined that a loose fuse clip caused the interruption in power.

The cedar creek service elevator was out of service from 10:05pm to 11pm on 1/25/07 due to problem with the e-stop bag by reset the elevator and put the elevator back in service at 11pm.

Alleged face ignition occurred: By statements received from the headgate shear operator, tailgate shear operator and shieldman; saw a small orange ball of flame lasting less than 4 seconds and self-extinguishing. No injuries. Sulfur in roof and face.

During the weekly examination, a reportable roof fall was found in F-Seam, entry 7, crosscut 76. The fall was approximately 20 feet long, 18 feet wide, and 9 feet high.

The fall didn't affect ventilation, travel, cause injury or damage to equipment.This area is not active workings. No one is required to work or travel in this entry.

The cedar creek elevator was out of service on 1/24/07 for 3:05am to 5:30am due to problem with the door not functioning properly. Bagby made the necessary repairs & placed the elevator back in service at 5:30am 1/24/07.

Roof fall occured in No. 2 entry near spad No. 725, 800 feet outby active face. Rock is 90 ft. long x 20 ft. wide and 6 to 8 feet thick. Area had some roof cracks with water seepage. Roof was bolted with full grout No. 5 x 48 in. bolts. Ribs had been timbered on 4 ft. centers.

An unplanned roof fall occurred in the 2 left #1 room set-up 1 x-cut inby survey station #288. Fall was approximately 30 feet long, 20 feet wide, 4 feet plus high. Did not affect any ventilation but did change the route of travel in the air course run. Area was timbered and dangered off.

The continuation of an existing roof fall was discovered in the 2 north #3 mains, 28 crosscut, #8 entry. The fall measures approximately 30' long, 18' wide, and 8 feet thick. The area is supported with 5 foot fully grouted resin bolts and 12 foot cable bolts.

An unintentional roof fall was found during a weekly examination on the 4th West return #6 entry at cross cut #182. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 18' long x 18' wide x 13' high. The roof fall will not be cleaned up.

A roof fall was discovered in the East Mains working section in Number 4 entry at XC 80. The fall involved the entire intersection, approximately 20ft W by 20 ft long, rib to rib and estimated to be 15 feet high.

Roof fall is approx 30' x 60' x 4' high. It fell into the intersection of No.4 entry blocking the primary escapeway. Fall area has been examined by MSHA and State. The primary escapeway has been re-routed and is now travelable. The area will be cleaned and resupported.

A rockfall was discovered during a weekly exam in an isolated area 2400' underground in the #4 entry of five entries. The fall had no affect on ventilation and/or travel and measured approx. 70' long by 20' wide by up to 5' thick. All entrances to the area were cribbed and dangered off.

A roof fall occurred on main north track at 25 xcut, 28+00 feet, #4 entry (old 2 west area). Fall was 60' L, 20' W, 8' H. Area was mined in Oct 1988. Roof fall occurred due to deterioration of the immediate roof around existing roof bolts. Area was supported with 8' resin anchored 2', expander shell roof bolts, plank, header block, & 12' passive cable bolts & cribs with crossbars.

#3 hoist tripped due to a spear point. After MSHA was contacted it was determined that the hoist was still available for personnel hoisting. Therefore, this incident did not meet the immediately reportable criteria.

An unintentional roof fall was found during a weekly examination on the 4th West return #6 entry at cross cut #179. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 18' long x 18' wide x 13' high. The roof fall will not be cleaned up.

A roof fall was discovered on this date 1/23/07 in the West Mains # 5 Entry between #11 and #12 Cross-Cuts. Fall was discovered by the weekly examiner and reported immediately. The MSHA call center & District #3 was notified at the time found. Could not determine exact date or time of fall. No Equip damage or injury. No disruption of ventilation.

The cedar creek elevator was out of service from 10:15pm to 11:15pm on 1/22/07 due to problem with the e-stop. Bagby reset the elevator drive system and placed elevator back in service at 11:15pm

An unplanned roof fall occurred at approx 1:00am on the #1 section in the #5 entry which is approx 400' inby spad #2437. The fall measured 20' long, 20' wide and 9' tall. No equipment was involved. Ventilation was not affected. No injuries were reported. Passage of persons was affected. All entrances to fall have been breakered off and fall will not be rehabilitated.

At 3:35/am a roof fall was found in the 1st Right section at a bore hole that had been originally drilled into the mine as a pilot hole for a raise bore. Because the area had been intended for a raise bore, there weren't any truss bolts in that area. The top fell right to the truss bolts on all sides of the fall.

A Fall of roof occurred on # 2 Unit in the 2A Panel, # 2 Entry, x-cut # 45. The fall was approx 20' long x 18' wide x 8' high. Cause was determined to be an undetected slip with water.

A Fall of roof occurred in the 1B Submain at x-cut 38 in the #2 Entry approx 560' outby the # 1 Unit. The fall was approx 100' long x 18' wide x 8' high. The area was supported with 6' fully grouted bolts as primary support and 8' cable bolts and cribs as supplemental.

The #8 elevator was out of service for a period of time exceeding 30 minutes due to the B-0 contactor burning up. The contactor was replaced and the elevator placed back into service.

Electrician determined need to change a problem contactor on operable production hoist. Mine Foreman was told to evacuate the mine. Evacuation was starrted, but was not completed prior to beginning component changeout. Inspector arriving on site was informed of the situation.

The 1085 hoist was inoperable for approximately 3-hours due to a power failure in the lower portion of the mine. The power outage was due to blast concussion that knocked out fuses from their holder of the S43 substation transformer. There was no damage to the hoist.

Miner was cutting in number 7 entry face. The bits hit the sandstone roof, creating a spark that resulted in an instaneous flash of light believed to be an ignition. There was no noise, air blast, smoke, or damage.

Fall happened in #5 entry of north mains section at 44 brk. near spad pad #19093. fall was 15'x10'x4' thick. Approx 12000' from the fall. No ventalation was blocked or no injuries. Developed 11-10-04 using 4' resign bolts.

Located on the 11-A longwall section, an alleged ignition occurred while afternoon shift was waiting to be relieved by midnight. A small orange ball of flame was witnessed by the tailgate shearer operator - lasted less than 1 second, self-extinguished. No injuries (sulphur in the roof).

An ignition occurred on the longwall face on the 8 right section at shield #85. The ignition occurred while an employee was cutting the face chain in the panline. The longwall was in the process of moving and the section was in a non-producing status. The flame lasted 2 to 5 seconds and was extinguished. No injuries.

An unintentional roof fall occurred on top of an Eimco 2810 continuous miner in the unbolted face of #7 entry approximately 35' inby SS3 3640 on 3-Left section. The roof material dimensions were approximately 20'L x 20'W x 3'H.

At approximately 3:20 AM, 1/17/07 the propane vaporizer building was observed to be on fire. The vaporizer facilitates a 20,000 gallon propane tank which services the Process Plant. There were no injuries and the fire was extinguished with only paint damage to the small metal building. Damage to the vaporizer has not been determined as yet

A fall of the roof occurred in the 3rd SE sub-panel at crosscut 17 in #2 entry on #1 unit, one crosscut inby the dumping point. The fall was 40' by 19' by 8' high. Ventilation, equipment and personnel were not affected.

Cutting coal face with rock band. Spark from rock caused coal dust to ignite. MSHA was notified and investigated. Water sprays and pressure were as required and bits on miner were in good shape. Ventilation was good in the face and last open crosscut. Localized area affected mostly on left side of miner.

Ore Skip load gate failed to close after dumping ore, skip traveled down and filled with ore, gate failed to close and skip hung in shaft. Miners had access to mine at # 4, # 5 and # 7 shafts.

A roof fall occurred at Crosscut 71, Entry 2, 1st North Return. The fall measured approximately 20' long x 20' wide x greater than 9' thick. The fall did not result in any damage to equipment or property.

Unintentional roof fall occurrd 40 ' inby Spad # 2803, # 5 Entry, 1st Right, 2nd North East Mains 003 Section, 600' out by 4-B Tail. Bolted with 6' torque tension bolts, 20' Wide X 20' Long X 6' High. Slicky sided big slip.

Strata: shale,mixed,wet. No coal rider seam present. Working on breakering off all entrances, Area will not be rehabilitated at this time.

Unintentional roof fall discovered at #5 Belt, #9 Block, #3 entry, in block inby station 3959. Fall measured approx. 10'H X 18'W X 35'L. Area was previously supported with 6' full grout resin rods. Developed 11/05. No injuries or property damage.

A fall occurred in the #5 heading of the 2 Northwest Mains section at spad 1562. This is approximately 350' outby the active working section. No equipment, no ventilation disturbed, no injuries. Fall will not be cleaned.

Roof bolter operators were installing tensioned cable bolts through some steel mats. The holes in the mats needed to be enlarged to allow for grouting the cable bolt. A gas test was taken at the roof bolt hole and no methane detected. When the cutting torch flame was directed toward the roof, gas in the hole ignited. A small blue flame was observed and extinguished within seconds.

Friction caused an ignition on the face of longwall panel 52 as the head drum of the shearer struck a hard rock intrusion located between the main coal seam and the draw slate. The intrusion was between 16 and 18 inches long and 2-6 inches thick. A yellow colored flame appeared to be the size of a loaf of bread and was observed as it lasted about 8 seconds by the shearer operator.

A Chemicals I&E Technician was sprayed in the face with a liquid assumed to contain condensated hydrogen fluoride. The miner was in the process of performing routine preventative maintenance on an instrument tap. The system that the miner was working on is expected to be under a vacuum under normal conditions. The injuries suffered by the EE resulted in his death.

Two EEs were installing a new receptacle onto a scoop battery. One of the conductors feeding into the receptacle was misaligned. Without wearing protective gloves, he attempted to align it by gripping it with a pair of insulated slip joint pliers. Apparently the insulation failed and a short circuit occurred causing an electrical arc which burned his left index and middle fingers.

The lever on the bobcat that is used for locking/unlocking the pin so that the operator can change from bucket to forks was frozen. He slipped, landing on his right shoulder resulting in a fracture.

The breaker lever was tripped and he couldn't reset the lever to the OFF position so he proceeded to open the panel door. Employee was lowering the top portion of a high jacker (man lift). He removed the 2 pins to pivot the top down and when it came down suddenly he said it pinched his finger between the cage and frame work. Employee has seen a doctor and will have fluid removed. Over-exertion in pulling or pushing objects

EE was pulling a curtain from out of some muck and back started hurting. Started missing work 1/31/2007.

Employee was closing airlock door on mantrip while performing as a foreman on the retum seals of the alna outby section. He caught left foot in door which caused him to fall and twist left knee. Previous knee injury in oct 2003 at other employment.

Struck by falling object

Employee was jackleg bolting when a rock fell and struck employee's nose causing a laceration which required sutures.

Struck against a moving object

Employee stated that he was riding in a man-trip going inside. The man-trip hit a bump in the roadway and he bounced up and hit his head on the canopy jamming his neck. Employee didn't miss work until 03/06/2007.

Over-exertion in lifting objects

The EE was installing a top-roller into position during a belt move. He was lifting the roller plus the weight of the conveyor belt. His position in relation to the weight being lifted caused a strain to a muscle in his lower back.

Over-exertion (Not Elsewhere Classified)

Employee was moving the continuous miner cable from the roadway when he twisted his right knee. He was seen at the health place on 1-8-07 and was sent for an MRI. On 1-17-07 the results of the MRI were: a torn meniscus.

Struck against stationary object

EE hit right shoulder on rib on 1/6/07. Worked until he had surgery on 4/6/07.

Injured was walking when he stepped on a ledge twisting rt. ankle. *Did not started missing work until 1/15/07.

Over-exertion in pulling or pushing objects

Employee pulled shoulder loading oil in a canister. He did not start losing time until 3/7/07 due to surgery.

Over-exertion in lifting objects

EE felt a pain in his lower back when he pushed on a hoist. Taken off work by his treating physician on 01/09/07.

Struck by... (Not Elsewhere Classified)

We were digging out the inside rear area of a conveyor for about 1.5 hours with rear side guards removed for access. EE motioned to start the belt. Within a minute of the belt starting, EE was struck by a bar which was caught in the winged tail pulley. EE suffered cuts on neck and forehead requiring stitches, and torn ligaments in his left shoulder (surgery).

Struck by flying object

Employee was grinding and a piece of metal flew into his right eye.
