A roof fall was discovered at 4 main 2x #6 Hdg indersection. 18' x 18' x 12' high. The fall was 2,000' outby section. Area was bolted with 6' fully grouted and 6' super twists and 12' post tensioned bolts. A row of cribs were set along each rib. No ventilation or transportation was affected. No persons or machines were injured or trapped.
An unintentional roof fall occurred at 63 crosscut on the 4C intake escapeway at station 3+50 entry #8. The fall did not affect ventilation or cause personal injury. The fall did fall in the primary escapeway but their was 2 entries and the escapeway was rerouted to the other entry. The fall measured 25 feet x 20 feet x 6 foot high and had been bolted with 4 foot roof bolts.
During a holiday weekend while the equipment was not running, a propane vaporizer that had recently been refurbished had a gas regulator fail causing a localized fire in the vaporizer. The fire spread to the second vaporizer before the fire department could extinguish the fire. Damage to equipment is estimated to be approximately $6,000.
Unintentional roof fall in intersection in #4 entry #11 crosscut in the set up rooms on HG #2.
A fall occurred on the 7th 54 belt line at crosscut # 174 in the S.E. L.O.M Mains C. There was no personal, ventilation or equipment involved (just belt).
While making weekly return examination discovered roof fall in return of 2nd north west mains 1 crosscut inby station #5297.
Employee found the roof fall on 11/26/2008. The fall is in the #2 entry between break #62 and 63. The roof fall is not on the active section. Roof fall is approx 40' long x 18' wide x 5'-6' thick. Roof faall has been timbered and dangered off.
A roof fall was found by an examiner in the P2-2 zone (a worked out panel) at x-cut 28 #1 entry to #3 entry. The fall did not affect ventilation and passage of miners. There were no injuries due to the fall.
The #8 elevator was inoperable for a periods of time exceeding 30 minutes due to a pinched wire on the door lock. No individuals were on the elevator at the time. The intake escapeway was rerouted. All individuals were notified according to law. The elevator was placed back into service at 6:50 pm
Rock fall between 2 to 1 Brake - 20ft wide - 24ft long 3' to 5' high.
An unplanned roof fall occurred on the 4th Main West Mains at cross cut 221 in #6 entry. The area was bolted with 6' and 10' point anchor roof bolts. The fall is in old works and will not be cleaned up.
One of our examiners found a roof fall during the weekly examination of the 4 Main North return. The fall was located in entry #3 between crosscuts 9 and 10. The fall was located in the entry that the examiners normally travel. The fall was timbered and the examination route was re-routed.
Rock fall occurred in the 3 right bleeder off 1 left off 1 East Mains. The rock fall was approximately 20' wide x 30' long x 5' high.
A Fall of Roof occurred in the 2nd Northeast parrallels, at spad 4 + 90, x-cut # 7, between # 7 and # 8 Entry. The fall was approx 25ft long x 10ft wide x 6 1/2ft high. Undetected void in the top was determined to be the cause of the fall.
A roof fall occurred in the #9 seam in the East Mains in the #6 entry in the intersection of crosscut #24. The fall measured approximately 20'x 20'x 9'. The area was supported with 6' tension rebar bolts, 9' mega bolts and truss bolts.
A small fire had started on the night shift but was quickly extinguished with a CO2 fire extinquisher. The burnt gear lubricant reignited and could not be extinquished immediately.
Fire occured at the Surface Electrical Shop with no injuries.
Rock fall at spad No.616. The fall is in an intersection in old works. This fall will not interfered with normal mining cable. The fill will be timbered and dangered off.
An unplanned roof fall occurred on the # 1 section from the # 2 entry to the # 7 entry 80' from the face.
The elevator at the preparation plant was out of service for a period of time exceeding 15 minutes with an individual on the elevator. All agencies were notified according to law. The electricians found a blown fuse, replaced the fuse, and the elevator was placed back into service. The individual was then able to exit the elevator in the normal manner.
Hoist failure due to hoist drive plan card (interface). Failed at 6:15am called MSHA 6:22am report # 1-58519631. Evacuation complete by 6:56am. Card replaced and hoist tested, fully functional by 9:00am, back to work.
A bounce occured in the LW belt entry from the face outby two crosscuts resulting in extensive floor heave.
Hoist operator testing production hoist prior to hoisting product. Hoist tripped on # 2 converter fault. Shut down # 2 converter, started # 1 converter, made several test trips, all fine. Did not evacuate.
A roof fall was discovered during a preshift exam. The fall was located inby the 2-Left-1 head drive which is located in the #13 x-cut. The fall consisted of laminated shale. The fall measured 20' wide by approx. 40' in length. The fall was cleaned up and the roof resupported.
An examiner found a continuation of an existing reported roof fall. The fall is located in the abandoned P2-2 panel. The fall is approx 15' high, 20' wide and 20' long. It was originally supported with 5' fully grouted resin bolts. There were no injuries involved and the air flow was not disturbed. The fall is not in the weekly examiners weekly route.
A roof fall occurred in the #9 seam in the Main North, #5 entry, in the pillar line between crosscut #20 and #21. The fall measured approximately 40'x 20'x 8'. The areas was supported with 6' tension rebar bolts and truss bolts.
A reportable roof fall occurred on #3 Unit, Crosscut 21, #2 Entry, the Crosscut from #3 Entry to #2 Entry was mined through. After mining this Crosscut, the unbolted area fell and extended into the #2 Entry, pulling out 7 torque tension bolts.
A rockfall was discovered in the south mains right return B5 #2 entry. The brow fell from the side of an already excavated overcast. This was reported as required and the MSHA Inspector on site was notified.
Roof fall occurred on the active pillar line on 001-0 Section while 3-cut pillaring. The cont. miner was completing right hand cut in pillar between Nos. 5 & 6 headings when rock fell fouling the head. Before miner was freed, a large slip fell on cont. miner, across #5 heading & into the intersection. The fall was in a slip & had not been detected prior to pillaring activities.
At 4:00 am a roof fall was discovered in the returns along the #2 Buckshot belt-line. Spad # 02252. All approaches to the fall have been cribbed or timbered off to prevent travel.
During a weekly exam of the airways on 11-21-08, a roof fall was discovered one break across from survey station # 1823 in the #1 entry. The fall measures approximately 210' long x 19' wide x 4'-10' thick. Fall was reported on 11-21-08 at 1:00pm on the 1-800 number.
AT 10:40 am an MSHA Inspector inspecting the mine saw a 2 inch steel braided air hose rubbing on the brow. He declared the Alimak unsafe to operate which is the alternate escape route. The Inspector called his supervisor and at 12:15 pm MSHA decided this was immediately reportable.
An unplanned roof fall occurred in the Coalburg mine on # 19 belt @ # 19 brk that measured 40' x 12' x 10'. No injuries were reported. The area was developed on 5-14-08.
A new rope had been installed on the North side of the #2 hoist. The hoistman was stretching out the rope after the installation. The conveyance was accidently set down on the bulkhead below the 5100 level causing a slack rope situation. No one was on the conveyance at the time. Several kinks were observed in the rope. The conveyance was brought to the surface.
IGNITION: The Continuous Miner was cutting coal in the No. 1 Entry on No. 8 Section and while cutting close to the roof, a frictional methane ignition occurred. The ignition lasted approximately 2 minutes and was extinguished with 2 fire extinguishers and a water hose.
A ROOF FALL OCCURED IN AN INTERSECTION AT STATION #328 ON #1 BELT. THIS FALL WAS 20' LONG BY 18' WIDE BY 6' HIGH. THE AREA WAS REBOLTED, SCREENED AND CLEANED UP.
Our examiners found a roof fall in the 4 West return during their weekly examination of the area. The fall was located in a crossut between entries 2 and 3 at crosscut 79. The fall was adjacent to the examiners route of travel but did not fall in either entry. The fall has been timbered.
A roof fall was found in the 2-1 x-cut at #217 block near spad #2298. It fell between intersections. 50' long x 17' wide x 8' high. It was mined 3 months prior. It was bolted with 5/8 x 60"" & 72"" fully grouted bolts. The area was timbered & dangered off. It will not be cleaned up.
The roof fell in the #12 lift after the lift had been mined and extended out into the entry in front of the #2 MRS. The fall pulled four bolts. A retriever was used to recover the MRS. The roof was previously supported with 5' Hy-Tech bolts. The fall was on 003 section 4-Right Panel between the #11 and #12 crosscuts.
Unintentional roof fall discovered during inspection. Fall location is #2 Belt, #6 hdg, 11 block, inby station 5102. Fall measured approximately 20'L X 18'W X 7'H. Previously supported with 6' torque tension bolts and was developed on or about 04-10-04. Overburden approximately 300'.
A roof fall occurred on the 3F Panel, #18 crosscut, #8 Entry. The fall is 19 feet wide, 20 feet long and 8' high.
Fall behind the return fanshaft in the number 2 entry area was cleaned and resupported. The material disturbed and caused some spot bolting that needed to be done. The area was checked by MSHA and the OMSL.
Foreman was traveling airways and found roof fall on 11/18/2008. Fall in #6 entry at break #32 from the surface portal. Roof fall is outby the section. Roof fall is 30; long x 20; wide x 6' to 7' thick. Area cribbed and dangered off.
ELECTICRAL SWITCH ON CONVEYANCE LOCKED BRAKES AND WOULD NOT RELEASE THE HOIST. ELCTRICIAN REPLACED SWITCH AND CONVEYANCE WAS BACK IN PRODUCTION. NO DAMAGE TO CONVEYANCE OR SHAFT. CALLED MSHA TO REPORT CONVEYANCE WAS DOWN, AND CALLED BACK TO LET MSHA KNOW - BACK IN PRODUCTION
A fall of roof occurred in the 1st Left panel, # 3 entry, at station 16+10. The fall was approximately 45 feet in length x 20 feet in width x 12 feet thick. The area was supported with 5 foot fully grouted roof bolts.
Our examiners found a roof fall during their weekly examination of the 3L4MN (5A)return. The fall was located at crosscut 46 between entries 2 and 1. The fall was located adjacent to the examiners route of travel.
An unplanned roof fall occurred 1200' from the face on #2 section in #4 entry 100' outby spad 5720. The fall measured 20' length, 20' width, and 6.5' in height.
On 11/16/08 two supervisors entered the mine via the production shaft. When they came out of the airlocks they noticed there was a burnt smell in the air. They noticed Three John Deere Gators and One Kawasaki Four Seater Mule had burned completly to the frame. The fire started and burned when no one was in the mine, and was caused by an electrical short
A frictional methane ignition occurred in the 8th East develpment section. The operator completed his final cut and was in the process of cleaning up when the miner bits contacted the mine floor, igniting a pocket of methane in the floor.
A frictional methane ignition occured in the 9th East CM section. Operator was turning a crosscut (15' depth) when the miner bits contacted fallen roof rock. The miner head was 1' above the floor when the ignition occurred. It appears that a spark was generated from the sandstone, subsequently igniting a methane floor feeder. The ignition lasted less than 10 seconds.
A roof fall occurred in the 2nd South in entry 3 at x-cut 18. The fall measured 20' long, 25' wide and 8 feet high. It did not cause injury, affect travel, ventilation or equipment. The fall will not be cleaned up.
The roof failed above the anchorage zone causing 3 roof bolts to fall out adjacent to the block side of the headgate just inby crosscut 14. The fall initially measured 4 ft wide by 15 ft long and was 11 ft high. The following day this area fell again resulting in a headgate blockage.
Fall @ x-3 21st Lt. return, #6 entry. Length 60', Width 17', Thickness 10'
Making preshift examination of beltlines and found a roof fall on #3 belt, one break outby #3 tail peice at spad #988, 3700 feet from face. 6 feet in height, 18 feet in width and 48 feet in length. No injuries to person(s).
Intersection fell from the face which was extened 10' beyond the last open xcut to 5' outby the last open xcut, Fall was 30' lomg x 17' wide x 8' high. The area was bolted with 5/8x72 fully grouted rebar and 12x12 plates. The following was dangered off and cribbed. It will not be cleaned up, the fall occurred between 5 lifts.
An unintentional roof fall occurred at Flint Ridge #2 mine outby the active sections on #2 main line belt line at cross-cut #12. No injuries, some damage to belt structure. The fall area strata is laminated shale and sandstone. Fall was app. 8 feet thick, 20 feet wide and 60 feet long.
An unplanned roof fall occurred in the 3rd southeast sub mains located 160' from spad #6181 and 11,680' from the section face.
Our investagation has led us to believe a gas pump leak combined with the heat of the engine started a fire in the engine compartment of the Cat Excavator.
Wet weathered shale seperated above roof bolt anchorage allowing roof to fall.
At around 10 :55 am the Hart street elevator failed to operate properly. The elevator cage brake dogs locked and would not release properly. This event occurred during an MSHA quarterly inspection of the elevator. The brake dogs were re-adjusted, greased and the elevator was returned to service.
An unintentional roof fall occurred in the 12 I section at 13 x-cut. The fall occurred between the track and the belt. The fall measured 8ft high by 16ft wide by 70 ft long. ***No injury or equipment damage
Foreman was traveling airways & found roof fall on 11/13/08. Roof fall was in the No 16 entry outby survey spad 1948. The roof fall is approximately 30' long x 18' wide x 3'-4' thick. This area has been dangered off & additional roof support has been installed on the inby & outby end of the roof fall to make area safe. (Roof fall reported to MSHA by phone 11/14/08.)
Intersection of 15E and 28S roof fell above the anchorage of the bolts. Intersection is in an area that was bermed to prevent entry.
A roof fall was discovered in the 2 west 4 mains # 39 crosscut in # 6 Entry. The fall measured approximately 30 feet long, 17 feet wide and 6 feet thick. The area is supported with 5 foot tension rebar bolts.
A rock fall appox. 20'x20'x6' high fell in the break between # 1 and 2 headings in the return near spad number 64. This fall does not impede the travel or air flow.
7 Mains 2, 26 block, 5 entry, MMU 001, Spad # 16382. Support: 6ft 3/4 inch resin bolts, screens, pans, dywidag truss bolts, straps, post & Big John jacks. 17 feet wide x 30 feet long x 7 feet high. Fall dangered off, additional roof support installed around fall and clean up plan posted.
Had roof fall on #2 section, #5 entry, 35' x 18' x 6'. Fall was 15' from active face in the intake on the section. No injuries.
While retreat mining on 002 section, mining last left lift, the roof fell and trapped mobile roof support at approx. 1:45 a.m. The rock was removed by a scoop and the mobile roof support was moved from the rock.
An unintentional roof fall occurred in the Main East Entries (001-0) off the Main North Entries in #2 entry at #13 cross cut +1140'. The fall was approximately 8' wide by 20' long. MSHA was notified within the prescribed time frame.

The Kuhntown Elevator was down from 9:07am until 11:30am. There were no employees on the elevator at this time. The elevator failed due to a loose plank switch. Switch was replaced.

During the normal pinion brake drag test, the hoist kicked out and would not reset. The pinion brake wear switches were adjusted & a broken wire reattached to the versa valve in south brake panel. Operational at 4:30 am.
Hoist was down due to fuze came out of holder on companies power pole.

A roof fall was found by an examination of abandon workings. The fall is located in P2-2 at x-cut 29 between #3 and #4 entry. The area was bolted with 5' fully grouted roof bolts. There was no injuries involved and the air flow was disterbed.
A roof fall was discovered in the roadway during a preshift exam. The fall measures 12-14' wide by 20' long and was 5-6' in thickness. The fall was in the 4-North Mains at crosscut #38 in the #2 entry. The area was previously supported with 5' HY-Tech bolts and truss bolts. The area will be cleaned up and resupported. An MSHA inspector was present at the minesite.
Location-east mains #5 entry #12 intersection. 8 ft long x 8 ft wide, 4 to 6 ft in height fall was a continuation from a previous roof fall in October 2005. Fall area will be cleaned up, rebolted and additional supports set in surrounding areas. No injuries no equipment damage. Mine was idle at the time of discovery.
Fire in the coal pile discovered at 7:37 a.m. This incident was reported to MSHA at 8:00 a.m. using the 800 telephone number and it was completely put out by 8:18 a.m. No person was injured and no equipment damaged was reported.
A fall of ground occurred in 4445 303 stope on 11/9/08. This was caused by a blast and didn't go past the zone of anchorage at the time after the blast. A 7000-1 form is being submitteed soley for the purpose of terminating a citation # 6441910 and should not be construed as admission that Newmont violated any MSHA regulation or that it concedes that a reportable event occurred.
No injuries, dust collector cartridge replaced. This was an unplanned explosion in the coal mill. Mill was designed to absorb and release energy and respond in the correct manner. No injuries & little damage
A fire broke out on the Cross-Pit Spreader between the 70-meter and the 20-meter. Inspections by structural experts revealed that a belt pulley possibly became extremely hot under the transfer point. The machine is not usable at the current time. Analyses are being conducted to determine if it is economic to refurbish or scrap the machine.
While retrieving structure in room set-up heard rock falling & found #6 room in 7th East butt had fallen in for 3 x-cuts. Built cribs & dangered off all access points to #6 room.
Discovered fall entry #2, 11:15 A.M. outby spad #20143, J-11 panel in return 20' X 19' X 6'. No equip involved, no vent affected, no personal injury. Area will not be rehabed. Area has been breakered off.
A fault above anchorage at break # 52 in # 3 entry. Area was supported with 5' fully grouted rods and additional combination bolts.

The natural gas boiler fired incorrectly resulting in a carbon monoxide (CO) emission in the main office. It was discovered by the presence of an odor. Immediately upon discovery all personnel were evacuated resulting in no illness or injury. Local fire department personnel assist in ventilation and source investigation.
A fall occurred in the 2nd west submain in entry #4 at X-cut #33. The fall measured 25' long, 30' wide and 7' high. The fall took out a stopping and caused some leakage. It did not affect travel, cause injury or damage equipment. The fall will not be cleaned up.
An unintentional fall occurred in the 2nd main west between #2 and #3 entry at the 5+60'. The fall was 8' high by 18' wide by 30' long. Area was bolted with 6' tension rebar bolts.
A fall of roof occurred in the South East Mains, # 2 entry, Station 28+00. The fall was approximately 40 feet in length x 40 feet in width x 10 feet thick. The area was supported with 6 foot fully grouted roof bolts.
A fall occurred in the second northwest parallel, number one entry, spad 2870-2940. The fall is eighty feet long, nineteen feet wide and unknown height.
Rock fall occurred on 1 right off 6 west panel in the #4 entry betweeen break #21 and #25. Fall measured approximately 20' wide x 280' long x 8'-10' thick.
A roof fall was discovered on old #1 section, In entry #5 at spad 489. The size of the fall was 40'x20'x91/2 to 10' thick. MSHA inspector and state inspector were at the mine when roof fall was found.
A roof fall was discovered at 3 main 53x#8 hdg intersection 18x 18x24. Fell above the anchorage of 6' super twist bolts and 4 @ 12' post tesnion cable bolts. approx 2,400' outby the face. no men or equipment were injured or trapped no ventilation or travel was impeeded.
16-W tail section. #3 entry, 69 block, 100' inby intersection a roof fall occurred. Length of fall 50', 8' high in middle of the entry.
Discovered a fall, entry #9, outby spad #17621 2:30 P.M. 50' X 20' X 7' high. No personal injury, no equipment, in return, no vent affected. Area was breakered off, will not be rehabed; MSHA & OMSL notified.
CO sensors in 2nd East Mains #1 entry indicated a possible spon com event. Persons dispatched to the entry discovered a heating at crosscut 13. Fire extinguishers and water were applied and the heating contained.
There was a frictional ignition on the 6-mains section. Miner operator loaded 1st S/C and made the connection from 5 entry to 5 to 6 x-cut at block 15. The operator was loading the 2nd S/C and was in the process of trimming the bottom when a frictional ignition occurred. The flash was orange/yellow in color and lasted a few seconds. Two employees were injured.
On 3-n sect. #4 inter, 4 BL. 100' inby spad 5138 Fell in approx. 15-20' high 16'w x 20'L top was gray shale laminated. Water was evident in top after fall. Cover of 270' area bolted with 6' resin bolts and 12' cable bolts. x3-4 was cribbed off also. Fall will be cleaned up and heizman canopy installed called MSHA hot line.
A ROOF FALL OCCURRED 1 BREAK INBY SPAD #257 IN THE NUMBER 2 ENTRY ADJACENT TO THE NUMBER 2 BELT APPROXIMATELY 20' WIDE X 40' LONG X 10' HIGH
Fall between # 17 & # 18 Break in # 1 entry. Approximately 60' long, 20' wide & 8'-9' high. Fall above anchorage of 8'
Two miners were preparing to start bolting the 1-5451 N sill when they noticed the left rib behind them dribbling. When the rib began to shift they pulled out of the area and evenually a slab approximately 3'W x 8'H x 10'L came down. The ground appeared to have failed to a slip.
Rock fall occurred on 1 East Mains in the #3 entry at break #35. Fall measured approximately 20' wide x 20' long x 5' thick.
The elevator governor failed causing one employee to be stuck in the elevator for approx one hour. Otis Elevator was called out to the site and fixed the problem. MSHA was contacted at 8:00am on 11-03-08.
Roof fall was found during the normal fireboss check. area approximately 5'x'5'10' on the West Mains #2 belt at #147 Break. Area has been breakered off and supportted.
A roof fall was found x21 #5 entry & xcut 18 to 19. The one in 21 xcut was halfway in the break 4 to 5 feet high & 20 feet wide & 20 feet long. The one at 18 to 19 xcut was 20' wide & 6' high & 50' wide. We will go around it at 17 to 20 to #4 entry & put new brattice in it. There will be 6 brattice.
While repairing a gate post, three employees, using a service truck and a Cat 950B loader, ""to reshape the post"". While the loader was moving into position its brakes failed and overran his intended point and an employee.
Employee was positioning idler on pallet and got left pinky finger caught between the idler and the pallet causing the end to be cut off.
Employee was operating shuttle car when he got against the rib and a piece of rock or coal came into the canopy and caught his hand against the grab handle he was holding onto resulting in the partial/full amputation of all four fingers on the left hand. Employee was flown from the mine site to St. Mary's Hospital by Healthnet Aeromedical service.
a piece of draw slate fell from a height of 9 1/2', striking the continous miner operator ont he right hand causing injury to the index, and middle finger. The piece of rock was razor sharp on one end and approx. 1 1/2"" on the other, measuring 18""-20"" across. Broke 1-2 fingers.
Employee was getting strap off top of miner. when miner boom was being sumped back foring strap into top and pinching hand.
EE was assisting in the removal of the canopy from a shuttle car, when the bolt was removed from the canopy leg, the canopy fell catching the tip of his left index finger against the frame of the machine, this resulted in the amputation of the end of his finger, the Dr was able to reattach the end of his finger.
While pulling core tubes, the EE was guiding wire line onto spool w/hammer. His gloves were pulled into the wireline drum where his 3 middle finger of (L) hand were caught in the cable wrap. I n a jerking reaction, the EE pulled back to free his hand and subsequently amputated his 3 middle fingers below 1st knuckle. At hospital, required amputation of middle finger to 2nd knuckle.
The employee was assisting inspecting the Joy 14CM15 miner MMU 005 #206 miner. When he pulled the hinge pin to drop the rub rail, the panel guard fell out of position and caught his right middle finger between the rub rail and guard. The incident resulted in a medical / lost time injury.
Employee was watching roof bolter advance up to next row of bolts to install with hand on top side of canopy. A rock fell 3' x 2' x 1"" - 2"" thick and struck him on right hand cutting his middle finger and severing his index finger.
Employee was walking and stepped on something that made his right ankle twist.
Employee was removing a liner plate from a chute, it became wedged. When the plate was released/freed it slid agains the employee's right hand.
Injured was unloading six inch aluminum pipe from a scoop bucket onto a flat car. He was pushing the pipe and felt pain in the left shoulder. He was off work on contractual time until 12/8/2008 when he saw a physician. He was taken off work at that time. The MRI report we were informed of on 12/15/2008 showed a tear in his left shoulder.
Elevated position (appr. three feet from ground) Injury jumped from the propel motor on a P&H 495 HR Shovel to retrieve a dropped wrench. He landed with his right heel on the track. His heel was fractured on impact. Swelling and bruising of tendons was associated as well.
EE was found by a supervisor lying on the mine bottom beside company # 41 jeep at 0 spur on the 4 Right longwall section. EE stated he was attempting to dismount the jeep, when he tripped on a bag of rockdust on the seat and fell, striking the mine rib. He attempted to use his arms to brace the fall and felt pain in his shoulder and upper arm.
Employee stepped off of manlift in the basement floor of the crusher. When he stepped off he twisted his ankle causing it to break.
While employee was removing belt structure, he struck his lower right leg with a belt stand. This caused a scrape and bruise to his leg. This resulted in an infection to the leg.
Employee states that while getting off loader, he missed the last step and twisted something in his back. Employee did not fall, but came down hard on the ground.
EE shut off the electric heater breaker, was in route to take the heater out of the primary crusher when he slipped on the snow on the conveyor belting that is on the ground in front of the panel box. He twisted his left leg when he fell.(aggravating an existing problem with his left wrist,needed surgery (3/17/09) by 220090980024)
Employee was climbing onto D10N dozer. His foot slipped on the track causing him to fall-back striking pusharm of dozer. He complained of tenderness and swelling in lower back area.
EE WAS WALKING IN ENTRY, STEPPED IN MUD HOLE AND TWISTED LEFT KNEE.

Employee was watching the digging taking place on the back section of the property, slipped on the ice and fell back, landing on his right shoulder.
EE was helping drag a shearer drum with a chain, when the chain broke and struck him in the head, face and left shoulder. Lost time reportable injury.

Employee was walking across the cool hill 2 belt undercast when EE stumbled over a rock and fell off the undercast. When EE hit the ground he landed on his W65 self rescuer, this caused bruising and swelling on his right hip.
Miner stepped wrong and twisted knee - working conditions did not appear to be a cause, just an awkward step.
Employee was getting down off the back of a bucket truck and slipped on the bumper. *Employee is on a scheduled 5 week vacation followed by retirement effective 1-31-09. Date returned is last day missed.
Employee and coworker were carrying a pallet jack downstairs when he felt a strain in his back. (Was not reportable until he had surgery on 3-26-09. Was on layoff status at the time.)
Moving 6"" vacuum hose to clean out the screw pit of ash. While holding/moving the hose with both hands employee lost balance & fell landing on left shoulder. This 70001 is being filed at this time because it has just now been determined that it was a work related injury.
Employee's right fingers were fractured and lacerated when they were smashed between a hanging bar and a cable.
Rock wedge fell on left index finger, causing cut, needing 10 stitches.
EE was walking down the stairs when her boot got caught causing her to trip and fall down. She was taken to the ER to be checked out. EE was taken off work for two two days.
This report is for compliance, contractor feels the accident was not fault of driver or company. The truck hauled in a load of dirty coal, bed staged out and load did not dump, passenger side broke loose, driver's side stuck. Unbalanced truck flipped over, truck was level, but when load stuck, driver's side wheels sank, adding to turnover. Coal came from another mine site.
Employee was climbing into dump truck, his foot slipped on bottom step and he fell onto his back. Low back pain, hip pain and muscle strain.
Employee was working with a crew of 6 people unloading bags of anfo and saran explosives from the container to the magazine container when he started to feel discomfort in his back. He continued working and thought that the discomfort would go away. By the end of the work day employees lower back ached. Employee sought medical attention on 11/28/08.
Associate was climbing onto a 980g loader. He was climbing using both hands but was wearing cotton gloves which are very slippery on smooth metal. He was on the second step when his hand slipped and he fell backward approximately 36 inches to the concrete floor landing on his bottom.
While changing out the holdback on 08 conveyor, EE was aligning the new hub and in the process of lining up the key way, cut the tip of his finger.
While winterizing conveyor system, EE was standing on top of 3100 lb counterweight while a co-worker raised it using a 20 ton crane. The cable from the crane severed causing the weight & EE to fall approx 10'. The weight came to abrupt stop causing EE to fall flat onto the weight & the 150 lb ball that was previously attached to the crane cable fell on his legs.
The welder was cutting a dragline dump rope. When it was cut loose, the released energy in the twisted rope whipped out hitting the left knee.
Employee was working on a guard and he lifted the guard and turned and felt pain in lower back.
Employee was walking from the mill clockhouse to the parking lot and was struck from behind by a fellow employee in his personal vehicle. He fell forward and cut his forehead and knee. His only injuries are scrapes and bruises.
EE was attempting to free up a steel skid hung up on the treshold of a walk thru mandoor at #31 1/2 wall, 3 north when the 3/8"" chain that was chained to the 3 north #2 belt drive broke releasing the sheave block that was under tension and he was struck in the right forearm by the sheave block. He sustained a fracture to
Employee walking on a catwalk tripped over piping running along side of catwalk grating floor impacting stomach with a water line valve at handrail level.
EE was stepping up on a bulldozer. His left foot slipped off the dozer, pulled a muscle in his right leg.
THE EMPLOYEE WAS MAKING PRE SHIFT SLIPPED AND FELL HURTING HIS BACK.
Employee grabed co-worker to prevent him from falling. In the process he fell backwards causing deep bruise of butt.
A bounce occured in the LW belt entry from the face outby the crosscuts resultings in extensive floor heave. On 5/5/09 the employee required surgery on his knee from injuries on 11/23/08.
Miner was filling the wet duster, when the fill hose came off and struck his left leg.
Was stepping off of bus when his foot slipped on the stairs. He fell back and slid to the ground on his back.
Attempting to remove a taper lock bushing from a large sheave. A weld on a pushing bolt broke when an airgun was applied. The spinning weld and bolt cut through his leather glove and into the palm of his right hand, which was placed at the base. The cut was approx 1 1/4"" long and 7 sutures were required. He was wearing all required PPE and had done a JHA.
Injured employee was helping the rest of the crew wind belt w/belt winder when belt winder slid out of scoop bucket - striking his right foot.
Employee was assisting in removing the drums from the continuous miner when the bar they were using in the railroad jack slipped causing injured to fall backward. He was transported to the hospital with a broken knee cap and miniscus. Employee had surgery on 11-26-08 and is a lost time injury.
EE was driving under a belt cross while pulling a fan with a toolcat. There was loose muck on the floor and he lost traction. He backed up to get better traction. When he was about halfway through, the front tires caught loose muck, which drove Employees head between a return roller and the belt.
While driving to service a drill, employee encountered a wet down hill grade. He lost control, hit a berm and laid the lube truck #23296 on its top. He received a contusion to the leg and hit his head. Non-work related issues prevented him from returning to work. Recently an IME came back on 12/15 and officially took him off work.
Employee experienced pain in his lower abdomen when he lifted a pump that was stuck in mud. Employee was diagnosed with a hernia. Employee started missing 12-02-08.
Employee was struck in the head with a piece of 3x3 angle iron. Long bolted at one end swinging in arc stricking EE.
Employee stated he had walked up to the Bolter to cut a few pieces of tie wire. The Bolters were installing bolts and as the injured employee reached to cut the wire, the bolter operator trammed foward and onto this fellows foot. Slight hair line fracture to his foot. Started missing time the next day 11/22/2008.
Miner in powder truck basket loading face round, stepped backwards and fell thru the basket gate that was not latched. Miner was not tied off with fall protection.
Rock fell of highwall and went through the rear window of dozer.
While hanging a line curtain in the #4 entry, a loose place of rock that was strapped with a 4"" metal strap felt from the top striking EE on his lower left back siding down his back and landed on his left foot, bruising his lower left back and the top of his left foot.
EE is having hernia surgery due to a strain to the groin. He was lifting dryer wear plates and strained himself. Started missing work 1/21/09
After finishing breakfast break employee was playing a joke on another employee and started running in the parking lot beside the office, while running a dog came running upon the employee, employee jumped above the dog and landed without balance in his right leg and broke one bone in his leg.
Truck driver exited truck and received injury to left ankle as his foot struck the ground.
Employee was standing with his hand placed on canopy of pinner when some material fell from roof hitting his left hand.
While stepping to the ground off his end dump truck, injured strained (pulled a muscle) his leg. Note: The company did not work on the four day Thanksgiving weekend.
While driving along a flat stretch of haul road, the operator lost control of the haul truck. The haul road was wet due to light snowfall. The truck skidded, the driver overcorrected and ran the truck up onto the dryer prairie beside the road. The trucks momentum caused the right tires to align and lay the truck on its right side.
Attempting to lift sheet of metal stepped off sidewalk onto another piece of metal, twisting his knee.
While roof bolting, he was attaching a lower drill steel to the upper drill steel. The hole was misaligned and he had placed the lower steel in the drill chuck. He then used manual force and got the steels partially engaged. He then used the bolters boom lift to finish. He rotated steels and because they were bowed, his left hand was struck. Laceration between index and thumb.
An overburden truck driver was exiting his truck for break time and while walking down staircase tripped on third step from the bottom. He tried to catch himself with left arm on handrail and injured his shoulder. Staircase was clean and in excellent condition.
He was installing a side rail for conveyor belt structure. The rail would not engage in the cradle on the stand due to misalignment. The side rail would not quite reach far enough into the stand's cradle. He grasped the side rail and pulled the side rail back. Fell out of cradle, dropped to ground. The tip of his right index finger caught between rail and cradle, caused laceration.
Employee received chemical burns to his neck, arm and a cut above his right eye when an explosion occurred as he closed the valve to a tote containing hydrogen peroxide.
Employee was walking back to his equipment and slipped on a patch of ice. He complained of pain in his Left Foot. He was transported to the hospital, by ambulance, where the x-rays showed a fractured bone in his left foot.
Employee was climbing over a dozer pusharm when he felt a sharp pain in his lower back. The employee fell backward to the ground. Employee had a previous lower back injury that may have contib uted to this incident.
EE was operating a battery mantrip, when mantrip struck a broken rail. EE was thrown against breaker boxes, causing him to break left upper jaw, break upper teeth and bruise left rib cage.
A cow was loose in the pit and several mine employees were trying to herd the animal out of the road. EE aproached the cow to move it and the cow ran at him. When EE ran out of the way he twisted his ankle.
Employee was working in the house structure of the dragline, when he felt pulling at back of left foot. Employee was referrred to a physician on 12/2/08 and was restricted from all work activities.
Employee was putting rocks in bucket to clean an area. He overfilled the bucket and lifted with no assistance and felt a pull on his lower right-side back.
Employee was hanging ventilation curtain with wire when wire struck EE in the left eye.
Employee 1 break across from 3-D belt head helping start the forklift. When he tried to boost the battery it exploded causing injury to his eyes. Taken to E.R. for treatment, Dr took him off work until follow-up 11/24/08.
Employee was using hard control settings to move main pump out of O&K shovel, EE was caught in machinery between the lifting boom and front bed of truck. Employee was air lifted to Conenaugh Hospital.
EE was emptying two 32-gal. plastic trash cans with wheeled dolly for transporting from the Lab. He brought the cans to the Burner Floor dumpster to empty. While lifting one, dolly detached and fell to the floor. He had to lift it (about 20 lbs.) to a height of 50"" to dump the can over the lip of the dumpster. He felt a ""twinge"", soreness in right shoulder.
He dropped a roll of white shrink wrap on his right foot.
While pulling up boards to lower mine floor, a board with a nail in it slipped from one miner's hand and hit another one in the hand. Punctured and broke index finger on left hand.
EE was repairing a water leak on #69 shield. He removed a keeper pin on a 13mm. High pressure line. The hose blew and the pressure in the line cut his left ring finger and middle. Ring finger has 10 sutures and middle finger has 3 suture.
Injury was spotting bolts along a rib where sloughing had occured. a rib roll approximately 1'x31/2'x1' hit ee in the left arm and side causing bruising and contusions.
Employee was lifting ram jack and the jack came off the chain and hit employee's left foot. **Employee started losing time again on 2/23/09 due to surgery**
Employee was standing on a rock when his foot slip and fell between 2 rocks causing a fracture to his L ankle.
Leaving work, employee slipped on black ice in parking lot. Fell to the ground and fractured two ribs.
While degreasing the roofbolter, the water hose blew apart and struck the employee on the left knee causing pain and swelling.
Man pinned between back bumper of the blasting truck and front bumper. The driver of pickup stated his foot slipped off of the brake and hit the accelator causing the truck to pin the victim between the bumpers.
EE was making copies at the copy machine and started to return back to her desk when her left foot caught the edge of the copy machine causing her to trip and fall. EE was taken to the emergency room and x-rayed and no fractures were found. Testing was done later due to shoulder bothering her and MRI results showed a hairline fracture in her shoulder.
EE was cleaning conveyor belt with rag, rag caught by pulley, taking left hand along conveyor.
Checking water pump, slipped on snow covered rock and fell hurting left ankle.
The injured was using a bar to clean loose coal away from the surface belt drive. The bar was caught in the roller and pinned his hand against the lower belt. This resulted in abrasions and lacerations to his right hand and fingers
EE was torquing sideframe roller bolts on the backhoe and reported soreness in his left shoulder. Under further investigation and physical examiniation that it was tendinosis. The incident was November 18, 2008 and was working full duty until his scheduled surgery for 04/28/2009
Employee was putting a foam filled tire back on scoop. When tire was being rolled into place by crew the tire overbalanced and pushed on his left leg hyperextending knee. This was in 2 South miner section.
The employee stated he was operating the continuous miner when a piece of rock fell from the roof approximately 3' wide x 9' long x 1/2"" to 5"" thick and struck him on the right side of body.
While performing routine boiler maintenance on one of the mine's two coal fired steam boilers, the injured welder/mechanic had the unit shut down for cleaning bottom ash/clinker out of the fire box. The mechanic dropped the cleaning tool into the fire box, causing hot ash to blow through the service door onto his face. Resulting in 1st and 2nd degree burns to his face.
Employee was cleaning up work area at the end of shift and steped on rock and twisted ankle.
Pulling a block out from wall with Kamatsu PC400 track hoe and cable. The shackle broke and 1 piece came through windshield, hitting operator in the head and went on to hit the back window.
EE was pulling waterline when he tripped and fell. He caught himself with his left hand and stoved his left shoulder.
EE was mounting the machine to operate it when he said he slipped and fell approx. 3ft to the ground landing on his back. No one saw the incident, he reported it about an later and said he was ok, but wanted to let us know about it. He finished the shift and then went to the hospital later that night.
Person was attempting to lower fire valve installed in belt entry. The pipe the valve was attached to broke and when doing so struck the person in the head. The result of this was a ruptured ear drum, laceration to the head and fracture of the orbital bones around the right eye.
The employee was assigned to build a stopping and after finishing the stopping she was assigned to build another stopping. She stated her back was hurting from building the first stopping. She was been diagnosed with degenerative changes in her back and neck.
Person was pushing rock into longwall face conveyor after gob out occurred at headgate. While doing so he inadvertently fell into the face conveyor & was pulled under rock in the panline as conveyor was stopped. Injuries incl. lacerations to head & lower abdomen, fractures to pelvis, lt elbow & wrist, rt hand. Accident was investigated by MSHA WV & Mettiki officials.

Miner was washing a mantrip. While removing floor mats and covers strained back.
Employee was removing bottom structure under beltline. When the belt structure fell on his left index finger that he had on the belt rail. EE received 10 stitches in his finger.
Maintenance employee reached his right hand in between the belt and the end pulley, while the conveyor was moving. The pully caught his glove and pulled him in.
Employee was installing a roof bolt in 2LT x-cut. When he pushed the bolt up a piece of rock was dislodged from the top(measuring 16"" long, 8"" wide & 6"" thick tapering to a point)& hit him in the face lacerating his upper lip (left side) & breaking one of his teeth. The injury required 12 stitches to close.
A small portion of the lower high-wall near the coal seam sloughed off. This caused the employee tending a six inch diesel pump in the center of the pit (approximately 50 feet from the high-wall) to turn suddenly and twist his right knee. He was taken to the ER and evaluated as having a sprained knee and released.
After completing a series of tasks including supersack bagging, driving forklift, collecting samples, and housekeeping, employee was climbing a flight of stairs when he reported feeling pain in his lower back.
Employee was walking and tripped on a floor grating that was slightly elevated. Employee's left foot got hung up on the grating.
On 11-15-08 Injury reported a injury when going thru mandoor at 2 drive his left knee popped. Employee completed report but did not miss any work or go to dr until 4-2-09.
Driver was unloading lime from a pneumatic trailer. The product line on the trailer plugged with material. Employee attempted to use a 1 1/2 inch airline to blow or clear the product from the product line. The airline was placed in the product line. When pressurized the airline flew out and struck employee in the right knee.
Splicing Conveyor belt slipped on structure and cut leg just above knee.
Employee was operating a 21sc shuttle car. He rounded a corner and had his knee hanging out. He struck against rib.
The empoyee stated he was assisting in the hanging of back-up fly curtains and was standing against the mine rib when a scoop ran over a lump of coal causing a piece of coal to shoot out from under the scoop tire and strike him on the left knee.
Ee fell off 12' ladder while repairing a door, He had severe pain in lower back and continued to work. On 12/27/08 He was cutting mine cars and felt pain in lower back.
Employee ws tramming feeder off of tailpiece. He crawled over a rock and hurt his left knee. Employee continued to work until 2/16/09, when surgery was performed on his knee. He started to lose time as of that date.
EE was hanging a high voltage cable when he felt a sharp pain in his abdomen. The ER Dr treated for a hernia and pushed the herniation back in the abdomin and referred EE to an orthopedic surgeon and released him to return to work. EE had follow-up with surgeon and went in for surgery on 12/2/2008.
Employee stated that he was removing rocks from the hopper and felt a snap in his side, he continued to work telling a couple of co-workers, but failed to tell his supervisor untill the following week, EE had been to Dr and thay are stating that he has abd. hernia.
Injury and other ee had bolted and 1 entry and were moving to #2 entry. Other ee had the cable reel on, while he was backing machine up as he spun around corner the cable came across the mine floor and took his feet out from under him causing him to fall on r. shoulder & r. wrist as he braced his fall causing a fracture to r. wrist
Employee was operating a Cat D-8 dozer on a ore stockpile. He doesn't know when the injury happened. His back was sore and having spasms on 11-16-08. He went to the doctor and was not allowed to return to work. He contacted his supervisor on 11-21-2008 about his injury.
Stepped out of storage trailer and twisted ankle.
While dragging a 3rd stage cleaner machine backwards, Injury walked into a cement step which caused him to fall over backwards injuring his lower back.
Employee was attempting to travel from one side of belt conveyor to opposite side. Employee walked across belt crossover & while climbing down or stepping down employee fell, landing on a belt turn buckle and his self-rescuer
Employee was struck by ram car and suffered a laceration that required stitches.
EE was operating shuttle car and heading to the feedback to unload. He was lashing the SC cable due to the anchor being one break inby the feeder. While EE was back lashing the cable, sprocket broke causing the reel to lock up resulting in the cable coming into the operator's deck striking EE's left arm and caused arm to strike the canopy.
EE was attempting to remove a drain plug from a 777 rock truck. He was trying to remove the drain plug by striking a wrench with a hammer. He missed the wrench and struck his hand with the hammer
While waiting for the employees shift to start the employee started cleaning up her reporting area. While using a shovel to move garbage, she felt a sharp pain in her hip. The employee went to the doctor and was not released to return to work because of the hip injury.
EE stated that she was working at the bench in the bay at First Aid. She was cleaning, filling and painting fire extinguishers. She stated that her right shoulder was sore but did not report it to her supervisor. She stated that it was bothering her still on Monday and she went to the doctor.
He was operating personal carrier and hit rock, jarring steering wheel out of hand hitting his finger.
Employee was retrieving a tool from the back of a Johnson Indutries personnel carrier and was struck by a combination of rib coal and roof rock. The material hit employee in the lower back and his legs, forcing him into the back of the personnel carrier. He sustained multiple fractures to the lower leg, crushing injuries to the right pelvis and right ankle.
Was struck by drill steel on outter right arm when steels lowered out of roof due to correction made when drilling hole. Suffered a contusion. Cause was safety awareness. Rules and regulations were not factor. Mining systems and equipment was not a factor. Miner proficiency was a factor. PPE was not a factor.
Employee was pinning when a rock fell striking his neck.
Person was miner helper on section. He bent over to pick up and throw miner cable against rib & when he went to throw cable he tripped and fell head first into the coal rib. He continued to work until 1/7/09 and is now off work because of pain in neck and upper back. First day off work for this accident is 1/8/09.
Employee was walking when he stepped on uneven pavement and felt pain in his right ankle.
Stated: He had his knee (w/knee pads) on top of a Ram Car towards the tram motors in order to check permissibility on the tram motors. His knees slipped forward slightly and he put his left arm down to prevent falling forward. He experienced pain in his left arm and shoulder. His regular doctor has him off work to determine source of pain.
He was drilling a bolt hole, steels became jammed in roof, he put his right hand on top of the drill head to check the suction, a drill steel came out of the roof and struck him on the right hand causing a fracture and laceration to the right middle finger.
Employee was walking on top of rail car inside loading building. Employee appeared to have fallen between two rail cars on south side of train scale.
Employee received burns to his neck and face when unloading acid.
Employee claims he noticed his right leg itching and numb, and rash on right thigh and swollen. He claims it may have happened when he was working on a motor and noticed small, yellow spiders in the area.
Employee was raising structure on #3 belt from previous belt move when a rock fell from top striking him on top of head & left shoulder.
EE was moving a shuttle care cable when s loose rock 6"" thick 2"" long struck him on his left shoulder.
The employee stated he was bolting top and had started to install the right side outside bolt when a piece of coal popped off the rib and struck him in the back and knocked him to the ground.
Employee was putting drill steels together, the insert was not seated. When the employee applied pressure the steels popped striking his right hand. The result of this was a fracture to the thumb and smashing the tip of his middle finger.
Employee was using 2 come-a-longs to align the top plate of the carousel so it could be welded. One of the come-a-longs slipped while employee's hand was under the plate and mashed his right ring finger requiring seven stitches.
Slipped in confined area (bin), bruising tail bone.
The employee helping to install belt in the belt entry and putting belt together was struck by a piece of rock measuring 2' x 1' x 2"" on the neck and shoulder. The rock fell from between two bolts.
While performing plant clean up, employee exited the bobcat and his right foot slipped off of the drag attached to the bobcat. When his foot slipped, he landed on his right knee.
EE was in the process of moving cables during a power move. He was on the 3rd step of the ladder when he was struck by the cables. He fell onto his left side fracturing 6 ribs.
EE was bending a roof bolt when he slipped and felt a pain in his back. The man continued to work untill 11/20/08 at which time he reported off.
Employee was replacing V-belts on collector. Four v-belts had been replaced and he was working on the fifth; when his left pinky became wedged between the belt and the motor sheave.
Employee was welding on a 777C Rock Truck Bed, and a piece of Slag flew up and struck the employee below the left eye, causing a mild burn below his eye.

EE was running the dozer and stopped to go to the restroom. When he returned there were some rocks laying next to the dozer and EE tried to move them and strained his back.
Miner alleged that he slipped while walking in entry way and felt pain in his back Was not reported to supervisor for two days later. The miner had been previously injury in mining accident in Utah in May, 2007, resulting a back injury. Investigation found this to be a reaggrevasion of already injured back. This is being classified as an occupational illness.
Employee reported this injury on Nov.14, 2008. He states he was hanging cable when it uncoiled striking him in the back.
Employee was bending over closing the drain on the vacuum system in the X-Ray sampling room and when he tried to stand back up, the employee said that he felt pain in his lower back and could not stand up straight.

Conductor mounted the cut of loaded railcars to protect the move to the hill, and as he mounted the railcar, pulled a muscle in his right shoulder. Employee was instructed by doctor to be off work and attend physical therapy for a potential muscle tear. Foreman was not made aware of incident until 11/30/2008.
EE is a forklift driver. He complained of lower back and leg pain and said it had been occuring all week but was worse at the end of shift. He was seen and returned to work. More pain occurred on 11/13 and he was placed on work restrictions on 11/14.
EE was stepping out of landing machine on the 16wt section, EE caught his right foot on the bottom of the canopy, felt a pop in the back of his right knee.

Employee was operating the continuous miner loading a ram when a rib fell striking the employee on the back of his right leg and knocking him into the ram car. He continued to work until Nov 18, 08 until which time his physician began treating him for an infection in the affected area of his right lower leg.
Employee got off diesel scoop. Scoop rolled back & caught employee between scoop and diesel generator. Fracture to pelvis.
An 8"" slurry pipe blocked. While trying to unblock pipe, material inside discharged with force splashing employee in both eyes.
Our employee was using a set of bolt cutters to cut some chain, he was using his knees to help aid in the cutter when his right knee popped out. EE has been working full duty in his regular job since the incident. He had surgery on 12-22-08, thus making it a reportable injury.
Employee did not report his injury until 11/12/2008. Employee states he got out of his machine to cut a piece of screen when he stepped back causing his knee to give out.
As employee was riding mantrip, he raised his left arm up to shield himself from the run through track drops. As mantrip travelled through, his left arm was pulled back over his head.

EE was operating the mobile fuel truck into position for fueling a Cat dozer & the excavator. As he was backing the fuel truck, the brakes failed & he backed into the right front of the dozer blade. The fuel truck sustained a small dent in the right rear bumper. EE stated on 11/7 his neck & upper back hurt & he was seeking chiropractor treatment. The ee is still being evaluated.
While using Grader 16M, operator backed over rock causing a back strain.

Employee was pushing belt over with scoop, when scoop struck rib, causing rib to roll off on his back.
EE was carrying a solid concrete block, when he stumbled and smashed in between the rib and the block.
The employee stated he was shoveling on the belt when he turned and felt pain in his back. He has been diagnosed with degenerative disc disease.

The miner operator was operating the remote control miner on the 6-Mains section. The first S/C was loaded & the miner operator made the connection from the 5 entry to the 5 to 6 X-cut at 15 block. The operator was loading the 2nd S/C & was in the process of trimming the bottom when a frictional ignition occurred. The flash was orange/yellow in color & lasted a few seconds.
Employee was walking in a low part of the section and walked into a piece of mesh. The corner sheet was hanging down. Employee had a zero hood on buttoned up under his chin. The mesh caught on his right ear and tore while trying to get it off. Laceration to right ear.
EE informed me at 2100 that he had injured his back getting on his equipment (992G) at work time. I then took EE to ARH (ER) for drug test and whatever medical treatment was needed.
Member was carrying crib blocks from scoop bucket to miner. While carrying cribs member slipped on a rock and twisted his right ankle. Member drove himself to hospital. Follow up with orthopedic surgeon on 11-21-08.

11/2/08 EE slipped in a drainage ditch that was located along the mainline treck. EE was walking on treck and had a rail roller in his hand, when he slipped the bar hit him above the left knee, EE thought he had problem with the knee from a past injury, 11/15/08 MRI showed a broken petella.
EE was pulling drill steel out of hole when he sprained his right shoulder.
While stacking bags on a pallet at the ER plant, his foot became stuck in the pallet. He fell down and sprained his ankle. Upon standing, he could not put weight on it.

While performing an emissions test under a pipe truck, the engine was revving and it slipped into gear rolling forward and spinning him under the truck.
Injured was pulling steel to align in head, hand slipped, got caught between pusher and finger while head was still going up.

Employee was attempting to cross over the belt when he slipped and landed on his right shoulder. He went to doctor on 11-3-08 then worked his normal shift. Missed work on 11-4-08 due to injury, took a personal day on 11-5-08 for a court date. Returned to work on 11-6-08.

While helping to lower crusher off blocks prior to moving it out of position, the crusher shifted and appeared to be tipping. IE and another employee ran from the crusher. IE fell and tripped on some rocks. He hit his chin and experienced some neck and jaw discomfort.
While removing wheelmotor off HT 188 with wheelmotor handler machanic was removing the air hose off top of motor. Mechanic placed right foot in path of drive wheel while the handler was in motion. The handler drive wheel rolled up part way on the right foot causing damage to the foot.
Employee was stepping out of tunnel at a slight angle. Left leg slid out as if doing the splits. He reports feeling discomfort and pain at that time, however, he did not go to the clinic until 12/4/08. therefore, this claim was not reportable until 12/4/08. He has a strain to the left knee.
EE attempting to step down from Ballast Loading Dock to track level(approx. 26"" verticle drop in elevation). Lost footing causing him to fall backward. Attempted to brace fall by extending right arm/hand to contact ground on loading dock. When hand contacted ground, EE felt pain in right arm. Completed shift. Saw DR. 11-25-08. Rx prescribed, X-ray & MRI ordered. Restricted Duty

Employee was getting back on tractor when he slipped and fell app. 6-7 feet off the tractor to the ground landing on his buttocks. Originally reported as minor first aid on 11/22/2008. After Medical evaluation on 12/9 it was determined that he needed further treatment.
IW was welding overhead when he felt a pain in his neck.
He was attempting to straighten the top tray covers on the stageloader to put cribs on. While moving the trays he felt pain in his lower right side. This happened on 11/21/2008. He bagan missing work on 11/25/2008 for lower back strain.

EE and other ee were replacing elevator buckets on the prep plant. As he let off the comealongs, a metal elevator bucket slipped-knocking him against a metal angle brace. The metal angle brace struck his right shoulder and right side.
Employee was working on sump pump using an oxy-acetylene torch to cut off a bearing. A flash occurred causing a ball of fire to come towards him. The ball of fire caused multiple burns to the face, neck and right forearm near the wrist.

Lifting wooden blocks to block up a truck that needed repair, felt a pop in his back and a pull in groin area resulting in a back strain and hernia.

Injured and coworker were moving an empty Con-Ex shipping container with two forklifts. The forklift the injured employee was operating started to tip and employee unfastened seat belt and attempted to jump free resulting in injury to his leg.
While pulling the hood open on the peterbuilt truck, felt a pain in his right shoulder. Hood was slightly stuck, so he pulled harder to get the hood open. Sent driver to to see nurse and recommended ice. Driver stated it is only 20 degrees outside, so every thing is pretty stiff on truck.
Employee was cleaning his work area at the primary crusher when he slipped and struck his leg. This caused an abrasion/hematoma. Initial check found no breaks. He returned to work with NO restrictions. On the 27th he reported to the hospital and was found to have an infection in the leg. He was given restricted duty at this time.
Injury was drilling the roof when I needed to change to the 2nd still. the first fell out from the roof striking my finger on the chuck of the roof bolter. This injury did not involve any aspect of compliances with rules or regulations, job skills or attitude.
Was receiving load in haul truck from the 16 shovel. When the first load was placed in the haul truck it jarred the employee. The second shovel placed jarred her more resulting in thoracic and neck sprain.
Slipped on conveyor that was on the ground. Hit lower back on framework, causing bruising and strain to the lower back.

Employee was using a wheelbarrow to clean up in the Modeco building. When the employee went to dump the wheelbarrow full of lime into the skidsteer, he slipped and fell. The wheelbarrow tipped over and spilled lime onto him. Employee had lime burn to right eye.
EE was in control shack, he radioed co-worker to come back to shack when he could. Co-worker went back to see what was going on & EE was in shack. He had knot on his head and scratch on right hand. He did not know what happened. He possibly blacked out. No evidence to show what happened.
EMPLOYEE WAS PUTTING A CONVEYOR BELT ON WITH A ROPE ATTACHED TO A PULLEY THAT WAS THEN ATTACHED TO A TRUCK TO PULL. THE PULLEY BROKE AND THE ROPE HIT THE EMPLOYEE IN THE FOOT/ANKLE (R).
Pull cord broke on motor. Employee made up a new cord, wraped fly wheel with cord, motor started. Cord didn't release from fly wheel, cord striking him many times in hand and leg.
While attempting to set a cable into the eye of a dragline bucket, he pulled back hard on the cable. This caused him to raise up & strike his hard hat on the pulley jamming his neck & back. He did not seek treatment until that evening. He was prescribed medication. After taking medication he had a severe allergic reaction causing a hospital stay & time off with restrictions.
While rolling up the hose on the recycle trailer he felt a pull on his upper shoulder causing his arm to become sore.
While lifting a 7' piece of I-beam with another employee. EE said that he felt something happen to his lower back.

injured employee was clearing a plastic film jam up from the handlemaker on the Hamer Form Fill & Seal Automated Packaging Machine. Somehow his gloved right hand became caught in the top seal belt assembly causing crushing/cutting off injury to his right thumb.
Employee was on knees hanging 4"" water line. Employee twisted and felt a ""pop"" in his left knee. Employee finished shift. Employee was treated the following day at Miner's Hospital, Spangler, PA and was placed on restricted duty.
Employee was walking on the #3 unit and stumbled causing him to fall to the ground injuring his right foot.

Employee was operating a roof bolter when a piece of draw rock fell from between bolts and struck him on the left ankle. He was taken to Pikeveill Medical Center & x-rays revealed a bone fragment that had been chipped off the outside ankle bone. He was placed on restricted duty effective 11/6/2008.
Employee was loading an idler frame into a service truck. Idler frame fell out of the truck and struck the employee on the left ankle.
Replacing the wire rope on 2-ton Maxwell, Manning and Moore hoist, removing twist from wire rope, he removed becket from electrical panel end of cable. Ran hoist up and down to relieve twist in wire rope. Ran hoist control from remote control box with e-stop. Fastened wire rope end anchors thru back of electrical panel near transformer. No mainline.
EE had his hand on work deck when a co-worker laid the jackleg down onto the EE's left hand fracturing his 3rd finger.
On second day of employment, the employee stated that while stepping onto the scale strained his back.
Lifting tail pulley on Unit 911 cone discharge belt (he pulled a back muscle).
The injuries occurred when the scraper operator struck a 1'-10"" drainage ditch bouncing the back of the scraper. The operator was able to complete the shift that day. The next day the operator had bruised rt. and lt elbows and soreness in the lower back and hips.
Employee was on ladder watch when he ascended ladder to assist work partner with tarp. The ladder slipped and he fell from the 3rd rung.

Suburban Propane Service Technician was repairing a propane dispenser tank. He started to remove meter when pressure in the line caused meter to blow off mount. This caused employee to crush left hand ring finger against tank support.
Employee was getting logded under p&h shovel. Operator of shovel swung around to put a bucket in back of haul truck. Shovel operator over swang and made on tact with truck bed. When contact was made it jolted the driver. Hurting truck drivers lower back and neck.
Employee was en-route to dump, when the vehicle hit a pothole, jarring the employee. Employee did not report the incident until 12/09/2008. Seen Dr. and was released without restrictions. 12/16/2008 employee had follow up visit and was given restrictions.
Removing lug nuts off of haul truck 491 with 1"" impact wrench when he felt a pain in his left hand. Resulted in sprain of left hand and wrist.
EE was looking for a bucket to collect a sample from a stockpile. He lifted an equipment rim to pull the bucket out from under it. His hand slipped off the rim and it fell on his finger crushing it. He went to the emergency room in Gallup New Mexico where he was treated. His injury required two stitches and a splint to protect his finger.
The production tech was walking and tripped over a rock, causing a twisting motion to the left knee.
This employee reported the injury on 11/29/08 but did not require medical attention until 01/06/09. His doctor prescribed physical therapy on 01/20/09. But physical therapy was not approved until 02/05/09 by the workers comp adjuster.
Employee was attempting to slide drag rail under shuttle car. He pinched his left index finger between the drag rail and the rib. This resulted in a broken bone.
Employee's shift began at 5:00 p.m. on 11/28/2008. Employee was operating a haul truck and was backing up to a shovel to be loaded. The shovel's dipper opened prematurely on the dove tail, causing the employee to get jarred.
Employee was drilling a hole for an ice melt spreader mounting bracket, the 1/2"" drill bound up when the bit broke through the metal plate causing the drill to spin. Employees little finger was caught between drill and frame causing a fracture of his little finger.
When EE was getting off the forklift to put cardboard on the pallets in the rail car, his radio caught on the forklift arm rest, causing him to turn odd and twist his ankle when he stepped on the ground.
Employee was crossing a wet and muddy ditch when he stepped into a hole causing the right leg to stop and his body to go forward injuring the right knee. Upon further evaluation by an Orthopedic doctor, it has been determined that a procedure is required to repair the torn cartilage in the knee.
While changing v-belt on crusher feeder drive come-a-long slipped off anchor point and struck victim on right shoulder.
Employee was replacing an idler on a conveyor system. The idler fell and struck the employee on the right hand.
Injured EE wsa adding the new bumper rails to scales and the injured EE was supporting the pipe by hand, when the opposite end fell off its supports trapping finger, causing a broken finger.
Employee was installing tires on grasshopper frame and strained back.
Employee was getting out of the manlift after installing a hopper plate. As employee was exiting the manlift, he lost footing and twisted his right ankle. Employee received trauma and pain to his right ankle. Employee was restricted.
Removing center drive line on 980 wheel loader. Drive line came part way out and caught on frame. EE reached in to move drive line into a different position. Came loose, left hand, middle finger, was in pinch point area between frame and drive line.
While extending his upper body out of the wall to clamp top of siding panel into place, the employee rolled over on his side quickly to move out of the way of panel being hoisted. The toe plate he was lying on contacted his ribcage in a manner that caused bruising & much soreness. X-rays were taken to check for broken ribs (none).
EE was at the store to pick (2) 12V equipment batteries when he picked it up, he said his back popped, and took him to his knees.
A Bridgeburg employee from the Cowansville Shop was removing the parking brake from a CAT 990 with Come-A-Longs. One of the Come-A-Longs and a nylon strap slipped, and the unit pinched the workers forearm. The worker went to a physician the following day and was put on lite duty.
Employee was working on the S-6 screening plant at the quarry. Stone was jamming up on one of the belts and to properly remove the material a cover needed to be removed. Employee went to place the cover on the ground, he twisted with the cover and when he twisted to lower the cover, he felt a pull in his back.
Employee was walking and stepped on a rock twisting his left knee. This caused a knee sprain.
Employee fractured two fingers when he placed his hand between a conveyor belt and an idler.
Emp was cutting wood for a waterline job. While emp was aligning the wood against the rail with right hand he bumped the wood. Hand slipped, saw blade caught shirt sleeve and cut the flesh to his right wrist and tendon. Emp had surgery to repair tendon and close the wound.
Employee was moving an overhead crane that had a 2 leg sling on hook. One leg of the sling hooked an original safety tie off cable (not used) located above the rod mill. The cable was attached to a hand rail and the cable pulled the hand rail off. The hand rail then struck the employee on the front of the hard hat, knocking him to the concrete floor.
Employee was taken to the Company clinic on 2/9/09 and was placed on restricted duty. The employee states that while placing a screen cloth, weighted at 15 lbs., into the bed of truck, he felt his right shoulder pull (Allegedly). Employee was diagnosed with a right shoulder strain.
Employee was transferring tram assembly to wheels. Injured right thumb on tram assembly.
Replacing sheet of steel liner inside the VRM using a come-along and prybar. The sheet shifted pinching mechanic's hand between steel frame and door frame.
The slide on the seat broke causing his back to jar.
Employee was opening a circulating valve on a pump tank when he felt a pop in his right shoulder.
Employee alleges while driving haul truck, he had pain in his neck and shoulder.
Employee was tightening bolts on a 36 inch flange, slipped off of pipe and fell 4 1/2 to 5 feet on the concrete below, causing a laceration to the left hand and a fracture to the right leg.
Employee attempted to stabilize a cylinder that shifted while hanging on a motor crane. When he did this, his finger was pinched between the chain and cylinder.
The injured was drilling a roof bolt hole, when a piece of draw rock fell and struck him on the right forearm. This resulted in a laceration, which required sutures.
Employee was raking coating from kiln when a piece hit rake pole knocking employee into handrail. Abrasion to upper right arm. Returned to physician 12/1/08 shoulder pain, restricted activities of no lifting over 10 lbs., unable to use power tools and limited use of Rt upper arm. Procedure for raking coating not followed. Note: Employee retired.
Employee states he was helping to replace a belt on the scalper. When they were replacing the guard he got his Rt. hand caught between the motor and guard. He had soreness to his hand. The Doctor put him in a splint and was given Prednisone.
Rerailing a 12B track mucker and pinched finger requiring sutures.
Employee was holding a loader tooth while another employee was using a sledge hammer to pound in the pin. The hammer glaced off the pin, striking the employee's left hand.
Scaling in the mine felt a sharp pain - told supervisor not known if it was an issue. Completed form and told HR in December. Went to doctor - his ortho surgeon on 1/5/09. Was put on restrictions at that time - so now reportable.
Employee was putting panels on the back of the boat to work on rudders.
EMPLOYEE WAS LIFTING A TIRE TO HANG ON TIRE CONVEYOR WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER.
Employee was working, kneeling down preparing to hook up the ""D"" ring on a sling to lift a bearing pedestal when the 10 ton hoist hook assembly struck him on the left side of his shoulder and head.
Move metal plate-strain shoulder area-accident occured on 11/20/08-did not seek medical attention until 11/26/08.
Injured was trouble shooting a power center. A flash occurred resulting in a burn to injured's right hand.
Employee was going to step onto the ladder of his 990H CAT Loader from flat concrete ground, when he lifted his left leg up he felt a pop in his right knee.
EE was pulling belt with channel locks and changed locks, slipped and EE fell into rib and hurt right shoulder. EE had to have surgery on 12/8/08. Returned to work on light duty 12/9/08.
Employee states that while manually unscrewing the joint, the pipe wrench slipped and his momentum pulled him forward causing him to slam his right hand against the top head motor.
Trying to extract a bolt from hoist drum with the hammer and bolt, when the hammer slipped off the head of the bolt and hit his little finger.
EE slipped on icy ground while draining washing/screening plant at end of shift. Lighting was marginal, but EE was aware of slippery conditions and did not spread available sand to aid traction. EE continued to work and saw physician on 11/25/08. At this time work restrictions were imposed. No lost time to date, diagnosis is pending MRI. Restricted work ongoing.
Employee was cleaning out a chute when a clump of material fell from the top of the chute. The material struck the employee on the back.
The Loader pulled into the shop and the mechanic went to support the bucket with a Jack to inspect the lower side of the bucket. The mechanic put the jack in place with the handle up and told operator to cut the engine and then went to fold the handle down and loader bucket drifted down and pinched the EE's left hand on the jack handle and broke the EE's left index finger.
The mine had recently changed resin suppliers and new boxes are fully glued. The miner used a razor knife to open the end of the resin box. The miner steadied the box with his left hand while cutting toward it with the knife in his right hand. The blade exited the cardboard and cut the left hand. The miner was taken to a medical facility and received stitches.
Using a pry bar to line up bolt holes on rubber hose for dredge pipe line, pry bar pinched right middle finger, required stitches.
Employee was using a pry bar to raise coupling on lighting gearbox. The pry bar slipped, pinching left pinky finger between pry bar and edge of gearbox. Incident was originally reported on 11/19/2008 as a minor first aid laceration. Became recordable after x-rays on 11/26 revealed fractured tuft of L pinky finger.
BUCKET ELEVATOR BELT BROKE AND FELL. A CRANE WAS USED TO LIFT THE BELTING AND RE-LOOP IT OVER THE HEAD PULLEY INTO HOUSING. EMPLOYEE DIRECTING THE CRANE USED NYLON STRAP AS A CHOKER. WHEN THE BELT HAD BEEN LIFTED 30 FT THE NYLON STRAP PINCHED THROUGH BELT CAUSING IT TO FALL. THE EMPLOYEE GUIDING THE BELT WAS STRUCK IN THE HARD HAT AND BACK BY THE BELTING AND BUCKETS.
At approximately 8:30 pm Injury descended the fuel truck to re-fuel a rock truck in the pit area. While walking to the rear of the fuel truck he stepped on a rock and twisted his left ankle.
Cold temperatures, excess amount of clothing; standing close to saw horse to make fine cut. Saw kicked back cutting thigh. Because of excessive clothing guard got caught in clothing not being able to release
Employee stated that he was pulling the pin that connects conveyor car to conveyor car on the highwall miner launch. He placed his right foot between the car and the car pusher. As he pulled the pin, the car pushed forward and pinned his foot between the pusher and the car.
Employee was cleaning chunks out of hopper for kiln feed seal and dust got in his eye.
Employee was operating a D10 dozer. He was ripping out several rocks and when backing up, the dozer slipped into a hole. This caused the dozer to jar, resulting in an injury to the employee's back.
Employee was working at the Recycle Plant and his left leg was pinned between two pieces of pipe.
EE was moving refinery bridge crane to an open area to work on hoist cable. As he was moving the bridge crane, he passed under a roof support beam. His left hand was on top of a control box, his hand became caught between the beam and control box, fracturing the tips of 3 fingers.
A belt clean up man called off work saying his arm hurt. He was taken to the doctor and he was diagnosed having tennis elbow and put on restricted duty.
Employee was holding one end of a flexible screw conveyor. The conveyor was cradled in his arms and held against his chest. The employee reached out with his left arm and felt a strain in his back.
Employee was walking to the truck door after putting antifreeze jugs into the bed of the truck. Overturned ankle on a clump of dirt causing left ankle sprain.
Was unscrewing the steel plate under the equipment, 3 bolts were loosened when the plate fell onto the employee's arm. the 4th bolt was rusted and did not hold in place.
Employee was changing cutting edge on loader, with assistance of a co-worker, he was removing the bolts. The employee had a hammer on bottom side of cutting edge holding the bolt up, the co-worker had an impact wrench on top to remove nut. When the co-worker pushed down on wrench, the hammer slipped. When hammer moved, employee's finger came in contact with the spinning bolt.
Using a jackhammer to clear a plugged chute, hammer slipped injuring his right index finger.
Unbolting flange to flex coupling from Dredge to pipeline. Flex coupling when disconnected, swung to side, bruising hand of EE between workbarge ""A"" frame and flex coupling.
Employee was moving a tool box; his left foot got caught under the box.
Slipped on 990 platform while opening access door. He caught himself jamming right shoulder
The employee was taken to the company clinic on 11/17, received 10 stitches and was placed on restricted duty. Employee tripped on a steel tube and fell on the dust sealing ring from a cone crusher resulting in a R-hand laceration.
