The main hoist went out of service at 4:15pm. Upon investigation a hydraulic line fitting blow out was discovered. At 4:40pm the men underground were evacuated to the surface of our #3 shaft. At 4:45pm the hydraulic line and fitting were repaired. At 4:55pm the MSHA 1-800# was called to advise of the situation. The hoist was placed back into service at 5:20pm.

#1 elevator was on the bottom and would not return to the top. The car was down at 1:45 am and no one was on the elevator at the time. All government agencies were notified according to law and all escapeways were re-routed. The car had a broken returning communication cable. The cable was repaired, tested and returned to service at 2:45 am.
A roof fall was discovered in the No. 6 entry inby spad no.1394. The fall measured approximately 18' x 18'x 8' high. There were no injuries with this occurrence.
The roof along the face at the tailgate end of the longwall fell into the pan. The tailgate is blocked.
#3 Entry (belt-line)- Around 2:20 P.M. on 3-31-11, A large piece of rock measuring approx. 16' wide, 10' high, and 2' thick, fell off the highwall knocking down part of the canopy and the Main Belt line.
While cutting a 3 way intersection, #5 entry into #6 entry. Completed. The top roof rock fell across #6 entry, pulling roof bolts. The roof fall measures approx. 16'W x 16'L x 8'H.
A roof fall was discovered in the 3rd Northeast submain #2 entry, xc-20, spad #16+80. The approximate size of the fall was length - 35ft, width - 35ft and height - 6 1/2ft.
Roof fall-5-D mains #3 Heading 30-Block Intersection. Located at station spad 2729. Approx. 17' W X 5' H X 20L.
Wet slick sided formation - unintensional roof fall occurred - 7800 feet from 001 mmu 30'long X 19'wide X 6'thick - fall is breakered off with 12'heizman jacks.
Elevator was down due to an electrical (control cable) problem.
An unplanned roof fall occurred in the 16th Left longwall tailgate located in the No. 1 entry of the 17-Left Return at crosscut #198+12, MMU-009-0 at approximately 6:30am March 30, 2011. No employees were involved. The tailgate blockage plan was implemented and in place March 29, 2011 at 12:00.
An unplanned roof fall has occurred on the #1 Section #4 Right Return panel spad 4553. The roof fall was 20' x 19' x 4'. No injuries were report or no equipment was involved.
On the 8 Shaft Hoist an electrical contactor failed, the hoist would not operate. No one was stuck on the cage or injured, the contactor was changed and the hoist returned to operation.
Roof Fall was discovered on 4 North Mains at 3:45 A.M. by Dwayne Maggard Size of fall 30'L X 30'W X 8'H. Fall does not effect ventilation or impede travel, roof fall will not be cleaned up
An unplanned roof fall at #26 brk. on #2 belt, #1 entry 20' into the cc.. 2 to 1. Fall was approx. 5' thick X 35'l. X 19' w. Area was developed in 01/11. Occurred 2000' out-by the section feeder. No entrapment or injuries involved. No EQ., escape way, travel or ventilation involved.
23 miners became trapped on the elevator at 6:08 AM when the safety brake on the bottom of the elevator tripped. The elevator was restarted and the men were off the elevator at 6:40 AM. The Shindler elevator team was called and responded to the mine. At 10:37 PM the elevator was cleared for operation.
An unintentional roof fall occurred on the Main West 1, Crosscut 43, between Entries 4 and 5. The roof fall measured approximately 25' in length, 17' in width, and 5' of thickness. A 103 (k) Order was issued March 29, 2011, by inspector.
At 10:25 AM the mine examiner found a roof fall in XC 38 North Mains #2 entry intersection.
A roof fall was discovered 3 x-cuts inby spad 4587 in the number 1 entry during the weekly examination. The fall did not affect persons, equipment nor ventilation. The area is dangered and cribbed off.
A fall was discovered in the old works 3 West Panel, the fall is appx 20 ft long x 20 ft wide, 1 break inby spad #573 intersection. The airway was not blocked. The fall was reported as required by law. The fall is dangered off and timbers are set.
A roof fall occurred in the 6-Left section outby in the #2 common entry (80' L x 18' W x 4 1/2' H). The roof had been supported with 4 1/2 foot tension-grouted bolts on 4' x 4' centers. Ventilation was not affected. No personnel or equipment was affected. Area comprised of sandstone channel/slicken-sided roof rock.
During a pre-shift a roof fall on the 002 section was discovered. Fall was approximately 20 ft. outby spad # 954 in the #4 entry. Fall was approximately 100ft. long, 18 ft. wide and 15 ft. high. Area has been cribbed off and supplemental support has been installed in the area surrounding the fall.
An employee got trapped on the portal passenger elevator at approximately 9:44 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The employee was freed and the elevator was returned to service at approximately 2:20 PM. A defective slow down switch approximately 80 ft. from the top landing was found to be the problem.
Elevator was down due to an electrical (switch)problem.
A fall was found 1 break inby 6069 in the No.6 entry the fall measured 70' long x 18' wide and was 12"" thick. The fall has been dangered off and will not be cleaned up. The fall was found by an EE and was reported by telephone on Sat 03/26/2011. This fall does not impede travel.
A roof fall occurred at the above mentioned time outby the # 3 section loading point in the # 3 entry in a neutral air course (not in a escapeway).Inby survey station 5561. The fall was approximately 40' long, 10' wide, and fell about 3' high. All approaches have had breaker timbers set and been dangered off.
Slope car was down due to an electrical (breaker) problem.
IGNITION: Employee had started to weld on a CM miner head when another employee saw a flame on the mine floor. It appears that a methane bleeder was ignited from the sparks. The ignition was put out with water from a wash down hose located on the miner. The ignition occurred outby the face of No. 4 Section. No injuries were incurred.
IGNITION: An ignition occurred on the No. 4 Section in the crosscut, between the No. 3 and No. 4 Entries. The ignition extinguished itself apparently, in that the witness stated they saw a flash while cutting.
A roof fall on the 002 MMU in the #7 entry 140 feet outby S.S. 1011. Fall was located between the #22 and #23 crosscut. The fall was approximately 20 feet wide by 20 feet high by 70 feet long. The fall is cribbed off and will not be cleaned up.
At approx. 10:50 the evening section foreman called out to report a fall in the #3 entry of th e3 S.W. 2 left panel (east Section) on the pillar section. The fall measured approx. 18' W. X 30'+ H. X 80' L. There were 2 MRS's and 1 14-15 CM Miner.
An unintentional roof fall was discovered in the Neutral Airway 1 break inby spad 1810. The fall measured approximately 25ft long x 18ft wide x 7ft high. No injuries resulted from this fall.
The door switch at the mine floor was fouled on the 8 haulage elevator from 12:01 am until 12:50 am.
IGNITION: An ignition occured on the No. 1 Longwall at Shield 132 and lasted approximately one minute. The ignition was extinguished using water and two fire extinguishers. No injuries were incurred.
An unplanned roof fall occurred 2 brks outby spad #6716 in the #8 belt entry outby the #1 section. The fall will be cleaned up according to the fall clean up plan which is posted at all approaches to the fall. The fall dimensions are 17 1/2' W X 50' L X 12' H.
Rock fall occurred on 3 East Mains off 1 North Mains crosscut break #47 in the #5 entry. Fall measured approximately 20' wide x 20' long x 5' thick.
This mine is in inactive/non-producing status. Barometric Pressure change with water blown from trap in SC #2 seal. Curtain torn down by rock. Safety Dept Personnel and MSHA Inspector found 5% methane. Men were immediately pulled and power turned off. Action plan put in place under the direct supervision of two MSHA Inspectors who were on-site.
#3 production hoist EMS #1 brake did not release due to a limit switch being out of adjustment. Two means of egress to surface were available to personnel underground.
A roof fall has occurred in the Old Works located at Double Sub-mains in the 3 entry, 2 left, break 31. No ventilation or equipment effected, the area has been dangered off. The size of the fall is approx. 40'L x 30'W x 5'Thick.
We had a roof fall. It was located at XC 11 entry 4 in 1st right. The area had been dangered off and cribs had been set on all 4 sides of the intersection before the roof fall occurred.
A rock fall has occurred in the #3 entry on 8 beltline at brk. 19, the approx. size of the fall was 10'H x 20' W x 28' L. No equipment or ventilation affected, the area will be cleaned and bolted with 6' glue bolts and 10' cable bolts.
A REPAIRMAN WAS IN THE 2ND SOUTH INTAKE CHECKING THE PHONE LINES WHEN HE CAME UP ON THE FALL AT X-CUT 22 ENTRY 6 AND REPORTED IT AT ONCE TO HIS SHIFT MINE MANAGER.
An unplanned roof fall occurred at spad #46815 off of the 18th X-cut from the track going the new way. The area was bolted with 5' torque tension bolts. The fall was approx. 6' H. X 19' W. X 180' L. The fall will not be cleaned up it was timbered and dangered off.
Roof Fall - occurred at intersection of cross cut #15 in the belt line entry. Involved in the fall was #1 Belt line (structure, water line and air line) It was cover with approx. 8-9 ft of gray shale rock. Cause of accident was geological conditions.
Individual located the fall while traveling in the area.
On a scheduled shaft maintenance day shaftmen were scaling in the shaft. A scale fell and hit a guide bracket beam braking it. There were two other shafts available for escape from the mine at the time of this incident.
Unit was in transition area of a fault and as they were mining 2 bolts fell out of the last row. 3' w X 4'L by 3' high.
Roof fall fell above anchorage in 4 south intake escapeway at # 2 & 50 of #3 room. Approx 16' x 18'.
Rock fall occurred on 2 East Mains return at break #24 in the #4 entry. Fall measured approximately 20' wide x 30' long x 5' thick.
A non-injury un-intentional roof fall occurred (outby) on 1 North Main (return) in #2 entry between cross-cut 16 and 17. The dimensions are approximately 40 feet long, 20 feet wide and 6 feet thick. Rod material is laminated shale. The area was initially supported with 60 inch fully grouted resin rods.
Rock fall occurred on 2 East Mains return at break #25 in the #5 entry. Fall measured approximately 20' wide x 30' long x 5' thick.
A roof fall occurred in the 6A Section. The roof fall was 15' long x 16' wide x 12' high. The roof in that area included sandstone channeling features and slickensided rock.The area was bolted on 4' x 3'centers with 6.5' resin-assisted torque-tension bolts
An unintentional roof fall occurred on the belt line between 72bk and 73bk on 2 South track/belt entry. The fall measured 20 foot in width 100 feet long and approximately 15 feet high. No escapeways or ventilation was obstructed by the fall. They were no injuries obtained by the accident.
The 8 Haulage Elevator was down from 3:45pm to 4:28pm due to a blown fuse in the output card.
In the 5190-4100N Access, the SW Nose Pillar had sloughed down and went beyond the existing ground supports anchor points. Mud seams are present in the area and are believed to have contributed to this occurence.
A roof fall was discovered in the 3rd Northwest mains at xc-30,#3 entry spad #21+00. The approximate size of the fall was length-16ft,width-22ft and height-6 1/2ft.
An unintentional detonation of an undetected charge of explosive material occurred while the Gradall scaler was removing loose rock from the back and ribs of the SW-2 face heading.
An unintentional fall occurred in the 2nd west entry's at the 27+30' of the #3 entry. The fall was 19' X 19' x 10'. The fall will be timbered off.
An unplanned roof fall occurred on Crosscut 69 Entry 6 Main West 1 Beltline. The roof fall measured approximately 30' in length, 20' in width, and 7' of thickness. A 103 (k) Order (8433152), was issued on March 16, 2011 by MSHA Inspector.

The elevator door switch became misaligned. This issue made the elevator inoperable.
Rock fall off of section, in primary escape way in #4 entry between break #13 & 14, catching high voltage cable, tracking and communication, 2nd phone line and life line. Ventilation still good.
IGNITION: The Shearer was cutting coal and rock toward the H/G when an ignition occurred at No. 36 Shield around the T/G Drum. The ignition occurred on No. 1 Longwall. They used 3 fire extinguishers to put it out. No injuries were incurred.
A roof fall occurred in the #9 seam in the 3rd East Sub-Mains in the #8 entry in the pillar line between crosscuts #9 and #10. The fall measured approximately 50'x18.5'x9'. Water and closely spaced limestone nodules were determined to be the cause of the fall.
Roof fall off section in the #5 entry on the 1 north main between break #12 & 13, will not be cleaned up

Galena Hoist 900 h.p. motor failed on start-up, shorted on rotor windings under high load, insulation failed.
A roof fall occurred on the mainline track at survey station 16-00. The fall occurred above the anchor point. The fall was approx. 30' L.X 16' W. X 9' above the coal seam.
A roof fall occurred in the #9 seam in the 3rd East Sub-Mains at the #3 crosscut in the pillar line between the #4 and #5 entries. The fall measured approximately 20'x18'x6'. Water and an undetected slip were determined to be the cause of the fall.
A roof fall occurred in the #9 seam in the 3rd East Sub-Mains in the #4 entry starting in the pillar line between crosscuts #13 and #14 and ending in the pillar line between crosscuts #14 and #15. The fall measured approximately 65'x18'x8'. Water along with an undetected slip was determined to be the cause of the fall.
Roof fall on the 002 section # 3 heading while getting first lift of the pillar block. Roof fall came from the pillar line, directly behind catching the #1 MRS as it was set up for roof protection. The approx. size of the fall was Length-30' width- 20' height- 5.5'. MRS system worked to catch falling roof and was retrieved after order was terminated to allow removal of MRS.
While checking for a possible plug in the posimetric feeder the unit 5 utilityman and I opened up the feeder to get it clear when the temperature spiked on the mill causing an explosion. No one was injured and there was no property damage that occurred from this incident. The explosion was contained within the coal mill.
Mine Foreman was making his exam and checking pumps(We were having heavy rainfall this early AM and the day before)When he encountered 11-12"" of water in the #7 entry/Intake Spad 768. All men were pulled back past the affected areas. Additional pumps were set to drain water to surface. Agency notified.
Operator was attempting to lift a preexisting tank, prior to the lift operator had miscalculated the load. The front stabilizer was not deployed and the lift was attempted from the side. The rigging attached to the tank was wrong. When the lift was made the tank twisted and the crane tipped over.
The Longwall 58 experienced heavy roof conditions in the headgate between 11 and 12 crosscut. MSHA told the operator this incident should have been immediately reported to the MSHA hotline. This report is in response to that decision.
At 1:06 P.M on 3/11/2011 an unplanned ignition occurred internal to K2 coal ball mill. The inlet temperature was not reduced to below 200F per shutdown procedures which led to the unplanned ignition. No injuries or damage occurred.
A cc located at 44 blk, 6 North between an int. air course and belt had been previously dangered off due to 2 potted out areas along each rib. The area is between a arch support and a permanently dangered off stopping. There is no reason why anyone should have to travel this area. The potted out areas have now further deteriorated to the point that we feel it may be reportable.
Roof Fall in 4 West 1 Right Section - #4 Entry Inby 7+02 to Outby 1 1/2 x-cuts to between 5+70 and 6+35, approximately 12' in height. No employees injured.
A roof fall occurred in the #9 seam in the 3rd East Sub-Main in the #8 entry starting between crosscuts #11 and #12 and ending between crosscut #12 and #13. The fall measured approximately 50'x18'x6'6"". A slip and water was determined to be the cause of the fall.
An unintentional roof fall was discovered on the Mains Section, #2 Entry, Inby Spad #6543. The fall measured approximately 60 to 70ft long by 18 to 20ft wide by 8 to 10ft high. No injuries resulted from this fall.

A roof fall occurred in the #9 seam in the 3rd East Sub-Main at crosscut #13 starting to the right of the intersection of #2 entry and ending to the left of the intersection of #3 entry. The fall measured approximately 45'x18'x6'-8'. A slip and water was determined to be the cause of the fall.
A roof fall occurred in the #9 seam in the 3rd East Sub-Main at crosscut #10 starting to the right of the intersection of #1 entry and ending at the left of the intersection of #3 entry. The fall measured approximately 90'x18'x6'-8'. A slip and water was determined to be the cause of the fall.
During the weekly exam of air counts a rock fall was discovered on 3/9/11 at 6:25am in the #4 intake air course. Approx 20' inby the mine fan. The rock fall is approx 10' wide X 8' long X 4' high.

Roof fall on 3rd NW Main spad 377. 20 X 20 X 5. Intersection old works, no air course or travel way blocked.
The elevator #1 was leaving the bottom landing as the car started toward the surface an EE attempted to open the car door. Elevator stopped approx. 1'above the bottom landing. Six EE's were on the elevator at the time. The electrician was called, lowered it and removed the EE's. at 3:55 p.m. The car was operated and tested and placed into service at 4:05 p.m.
A roof fall occured in the B-10 Tailgate section in the #2 entry at the outby edge of crosscut 18. The fall extended approx. 24-feet outby, 18-feet wide and 8-feet high. The fall occured in wet mudstone roof.
A roof fall measuring 70' Long, 18' wide, and 9' thick was discovered in the #2 entry 7 right off SE Mains at spad 66193. The fall was 500' from the working face, ventilation or the passage of men was not affected. The fall occurred due to laminated rock above the 60"" torque tension roof bolts. The area was developed Feb 2011 no injuries resulted from the fall.
AT APPROX. 2:45 AM ON THE MORNING OF 03/08/11 THE AIR BRAKE VALVE THAT OPENS AND CLOSES THE BRAKES ON THE HOIST FAILED. THE REPAIR TOOK APPROX. 50 MINUTES.
While removing salt growth from level one entrance to the #2 shaft, an electrical control wire was damaged. The mine was evacuated at 9:41 pm. The control wire was repaired and the hoist was tested. Miners returned underground at 11:00 pm.
The #8 elevator was out of service for a period of time exceeding 30 minutes due to bad bottom door lock motor. No individuals were on the elevator at the time . All government agencies were notified according to law. All escapeways were re-routed. The elevator was placed back into service @ 11:00AM. after the bottom door motor was replace.
A roof fall approx. 40' inby the highwall face of the #3 beltline entry. It's approx. 20' wide 50' long and unknown height.
A roof fall occurred on the 3Lt.Mains Section in the #7R crosscut 1 break inby spad #422 at approx 7A.M.The fall continued outby into the intersection of #7 entry causing 3 rows of bolts to be pulled.The fall measured 25'L X 15'W X 8'H. The area was bolted with 4' fully grouted glue bolts. The area has been cleaned up and rebolted with 5' torque tension and 9' cable bolts.
A crew working in the old 2-H Panel located this roof fall and notified the responsible person. The area was dangered off and breaker post were installed around the fall area
Flint Ridge Mine #2 has experienced a non-injury, unintentional roof fall outby the working section. The fall was discovered on #1 Main at cross-cut 52, #9 entry/intersection on return. The fall is app. 20' long by 20' wide and 5' thick. Laminated shale had deteriorated. The area was initially supported with 48"" fully grouted rods.
A roof fall was discovered in No. 1 Entry of 3-Right off 4 Northeast Mains and was one (1) break inby survey station No. 3654. The fall was approximately 20' wide x 20'+ long x 12' high and was in the last open crosscut. There were no injuries or equipment involved.
In the intersection at brk. 41 of old #4 belt in the #1 entry. The nearest spad to this location is #1590. This spad is approximately 80' outby the fall.
During cutting and welding tasks on the longwall face, there appeared to be some form of gas burning under the panline that was quickly extinguished.
Roof fall neutral entry #2 entry 3 belt Spad No. 1280-1289, this fall is an extension of an older reported fall.
An uncharted oil well was mined into in the #5 entry of the 43xc on the 1st South Submain. Mining was ceased immediately and all equipment was de-energized. After the initial mud from the well, water started coming out of the well.
The fall was on #5A belt brk.#3 one brk. outby spad #1361. The fall was 18 ft. wide, 48 ft. long and 5 to 7 feet high. The fall was of grey sandy shale material. The area was timbered and dangered off. The intersection will be cleaned up and re-bolted, with standing support such as L.Lock cribs, super cribs, reg. cribs, 50 and 100 ton jacks.
The 8 Haulage elevator door mechanism malfunctioned causing the elevator to be down for a period of 36 minutes.
We were on an intersection trying to relieve some bags in the wire and re-bolt when a 10'long x 8'wide x 6' high section of the back came down. We were in the process of relieving the area at the time.
The fall was approx. 40'X40'X6' deep. There was slight damage to the elbow of the mine fan and to a electrical panel. This fall partially impedes airflow in the mine. The mine was immediately evacuated after discovery and MSHA was called.
A fall was discovered at #29 Block, A Sub (#5 Entry, 3700' outby the section). The area was bolted with 6' SuperTwist bolts and 12' Point Tension Cable Bolts. No equipment, ventilation, or individuals were affected by the fall.
A roof fall was discovered in the drift mouth of the No. 2 Entry (survey station No. 2) on the old CJ & L mine portal. This mine is now a part of this active mine. The fall measured app. 15' wide X 15' long X 4' + in thickness and was composed of laminated sandy shale.
Discovered a continuation of the roof fall that is in #6 entry, 2 breaks outby spad #2992. The continuation of the roof fall is 6' thick x 20' wide x 15' long. A rehabilitation plan will be submitted for MSHA approval.

The fall was dangered off and no persons were affected. The area was originally supported with 5' Hy-tech bolts and 12' cable bolts.
An unplanned roof fall occurred in the return along #2 belt in the #2 entry at spad # 260, 3000' from the active surface.
Fall was found on 1st left off of 1 west mains old track entry. Fall was 75' long x 9' thick x 20' wide. 14 break track entry. roof support was 5' torque tension bolts.
The Crabapple elevator was inoperable for longer than 30 Minutes.
Discovered a roof fall in the #6 entry, 2 breaks outby spad #2992. The fall is approximately 3.5' thick x 10' wide x 10' long. A rehabilitation plan will be submitted for MSHA approval.
XC 40 of the North Mains #2 entry intersection a roof fall was discovered by the examiner.
Fall was found off of 1 west mains old track entry 5 break. Fall was 5' thick x 20' long x 20' wide. roof control was 5' torque tension bolts.
Setting miner up to take first cut of coal when he became pinched between miner boom and rib.
Victim was part of a crew that was welding hdpe pipe using a McElroy fusion machine. While attempting to position pipe in to fusion machine, using an excavator, the pipe struck the victim in the head.
Employee was stacking cement sacks when he felt slight pressure on groin area.
Employee stated that while fueling equipment, he was dragging the fuel hose in the mud when he felt a sharp pain in his lower abdomen. He did not go to the doctor until 3/30/2011. He stated that he had a Hernia but did not and has not had it repaired. This is a Partial Permanent Disability.
An employee was working on the face conveyor chain of the long wall when the stored energy of the chain released striking the employee at the ankles, amputating the right foot above the ankle and compound fracturing the left ankle.
Employee was strapping load down to the truck and a large rock split in half and fell on the edge of the truck. They called for help and before the help arrived, the employee failed to clear the truck and the rock fell on the employees foot.
While placing a bushing into the table on an angle hole the bushing slipped down and the employee's middle and ring fingers went between the bushing and the table. The driller activated the hydraulic cylinder to close the table and pinched the employee's fingers amputating the tips.
Employee was dropping a cable tree and the locking rope pulled his hand into the pulley causing an avulsion to the left 5th finger.
While pulling a cable using a rope and stinger ride, the injured employee caught his left thumb between the rope knot and stob on the ride. The injury caused his left thumb to be amputated from the point of his thumb knuckle.
Wet screen threw a belt; belt & guard were replaced.3rd time belt started to come off. Stopped & locked out screen but centric stopped on the top. EE reached into grab the belt and the centric moved pulling his (L)hand into the belt. EE grabbed the belt with his (R)hand to release his (L) and the centric moved again grabbing his (R)middle finger & ring finger.
EE was cleaning up tools, He was packing a 8"" Grinder he placed on a load bed the cord on the grinder wrapped around his tool belt pulling the grinder down. It fell and landed on his left hand pinky finger. Partial amputation.
On the right handle cover - pulled cover outward it fell to ground and pinched fingers between handle & cover.
There was no witness to the accident, ee was struck in the face by the metal concrete pipe to pour concrete on the slope floor. Were cleaning out the excess concrete in the pipe at the time of the accident which includes inserting 2 foam balls into the top and blowing them down the pipe. When the pressure relapsed it appears to have kicked back.
Repairing leveling chain on opposite side roof bolter. Mechanic went to stand up and fell forward, struck hand onto boom arm to hold himself up from falling into arm of bolter and his fingers(middle & ring) got caught between 2 boom arms cutting the 2 fingers off at 2nd knuckle.
CONTRACTOR EMPLOYEE WORKING ON DEMO SITE, WHILE PREPPING STRUCTURAL BEAM FOR TEAR DOWN THE BEAM UNEXPECTEDLY TWISTED HITTING THE EMPLOYEE'S LEFT FOOT. BEAM CRUSHED EMPLOYEE'S LEFT FOOT TOE AREA.
Employee had completed drilling his lifter holes and was out cleaning out the hole to install his lifter pipe. A grape fruit size piece of rock fell from an undetermined section of the face and struck the employee on the back just below his shoulder. Face had been visibily inspected and washed prior and during installation of collar pipe.
Employee was making a splice on the 6D belt. While lifting a piece of 48"" top structure, employee felt a pain in the groin area. Employee immediately visited Logan General where he was treated and released with no restrictions. Employee continued to work regular duties up until 7/14/11. Employee had surgery on 7/14/11 to repair a hernia that possibly resulted on 3/31/11.
Bruised right knee from kneeling too long on screen doing repairs.
Employee was going down the steps into the pit and a bolt had fallen and gotten lodged in the step and he did not see it and stepped on the bolt and twisted his ankle.
EE was welding a 12""X1""X3/4"" piece of metal onto a mill gang cart. He was holding the piece in his R hand and welding with his L hand. He stated he heard it pop and then the welding wire bounced up and penetrated his welding glove and R thumb. He applied 1st aid and continued to work. On 4/3/11 it was swollen/sore, went to ER, did 1st aid. On 4/4/11 got worse so he was admitted.
Employee was lifting 50lbs bags from a high lift to a rework station. As he was twisting around to move the bag, he felt a sharp pain in his left lower back.
ACCIDENT - Employee went to pick up a jack from manbus with hand and felt pain. employee saw dr. and was advised he needed surgery. Employee elected to put off surgery to later date and has been working up to this date. SURGERY - Employee had surgery 12/29/11.
Continuous Miner was turning Left Break in #5 Entry. While loading shuttle car a piece of rock came off of the boom of the miner and struck the employee on the right side of his face. A laceration on the right cheek bone required 4 stitches.
A mechanic EE experienced back pain while working on a track dozer
EE stated that while operating a red ride, he ran it into a table located outby the face area, injuring his hand (started missing work on 4/5/11).
The employee was exiting an excavator, climbing down the tracks to the step. He slipped from the step and fell to the ground landing on his lunch box. There was, according to other employees, some mud on the step. The employee was angry in leaving his equipment, due to the fact that he was to quit operating one piece and start operating another.
Employee was straightening out 500MCM Wire. Afterward he noticed a bulge in his abdomen area.
The miner was changing an oil filter on a haul truck. When the filter released from the truck the miner pulled a muscle in his groin. No damage or impairment.
Employee was working on the beltline in the tailgate section of M-Class mine when he was attempting to ascend belt structure. Employee suffered a dislocation of his left knee.
Inattention by worker and loader operator. Back of worker's foot contacted by loader bucket.
Employee was in a mantrip that was sitting still when another mantrip ran into them.
Employee could see the steam coming off of the diesel pump and knew it was not pumping. Cold water running into the hot pump case created steam pressure that exceeded the hose clamps capability and it blew off the suction hose. Hot water then spilled out of the pump case and splashed onto the employees legs.
Employee was helping hang miner cable in #9 entry when he re-injured his shoulder.
EE was moving roof bolt resin when a rock fell and struck him on his head and back. He returned to work on 3/31/11 but did not return to work on 4/4/11 due to injury.
As climbing down ladder slipped on ladder rung. Caught himself with right hand felt strain in right elbow and shoulder.
Haul truck was being loaded with spent material, there was some frozen chunks in the material. The loader dumped the bucket of material in the truck box and a large piece of ice hit the offside of the haul truck jarring the truck operator. When the operator was jarred he experienced back pain, the operator was recently released by a physician for a back injury unrelated to work.
The employee was weighing 50lb bags. When he stepped backwards, he stepped on an uneven area and lost his balance and fell on the surface he was standing on.
EE was pulling back the long wall face conveyor when he pinched his left foot between the shield pontoon and the relay bar. EE experienced a fractured big toe and a laceration.
Employee was working on belt when he slipped and fell and hurt his upper back. This happened on 3/30/2011, but he didn't tell his foreman until 7/25/2011.
Employee was driving his parts truck on hwy 160 when the power steering belt broke and the truck veered into the guard rail. This caused EE to hit his left wrist on steering wheel.
Employee claims that at the end of his shift he was getting off the water truck when he fell. He got up and walked off to his van pool never reported any accident to the mine. hip fracture. Hospital contacted ASARCO today with a verbal phone call. There is no evidence supporting that the accident occurred on property.
Operating a Hitachi 1700 Rock Truck traveling up an incline to the dump. As the truck approached the top section of the ramp, a large rock shifted causing the truck's front wheels to come off the ground and then back down, jarring the member. He returned to work, then on 4/1/2011 he was taken off work until 4/11/2011.
Employee allegedly strained a muscle in his back while hanging scoop charger cable. * Employee did not begin to miss work until 03-31-11.
Employee was climbing up stairs when his boot caught on some steel treading causing him to twist his knee.
The employee was lifting and pulling on a water pump mounted on maintenance diesel vehicle. He is diagnosed with rotor cuff damage.
Employee was bent over moving coal and dirt away from core holes with a rake, when he developed continuous pain in his neck. The pain worsened on 4/04/2011 and he went to the doctor.
Employee was crawling down the belt on lolo and his foot must have slipped off the side. They were in the process of setting bits in the miner and when they backed the miner up to roll the heads it hit the Mobile Bridge Carrier (MBC) causing it to twist and rubbed employees foot.
Victim was using a shovel to fill blast hole. When he stood up his lower back began hurting. Worked rest of shift. When he got up next morning it was still hurting so he went to Doctor. Gave him a steroid injection. He returned to work on 4-1-11.
Employee approached a haul truck parked at the ready line. He lifted his left foot to step onto the lower step of the access ladder, and felt a `pop' in his right knee. He immediately stopped on the ladder, and waited for the situation to feel better. After a few moments, he continued on up the ladder, and completed the work on the door, and then exited the truck.
EE stated that he was in process of turning the drill carousel with a slate bar when he felt something ""pop"" in his lower back. He sustained a strained muscle as a result.
Hydraulic fluid got into the eyes of the forklift operator for the warehouse. After taking several moments to wash his eyes out with eye wash and water, he deemed it necessary to seek medical attention because of prolonged irritation.
Using compressed air to get bulk cement flowing and when it did, it got into his eyes.
WHILE PULLING OUT AN AUGER A PIECE OF ROCK FELL STRIKING HIS LEFT HAND
Victim was hauling shot rock with a 777 Caterpillar truck. He hit a depression and it bottomed out the seat and his back started hurting. He went to Pikeville Medical Center ER later that day. Employee still off work.
EE was driving a shuttle car transporting coal from the cont. miner to the section dumping point. He ran over a bump or ran into a hole in the roadway which caused him to be bounced into the car's canopy causing pain in the back.
While working in the motor compartment of a rail mounted jeep, the compartment lid fell on his right hand causing a laceration.
Installing the 3 lower sections on the 3B classifying cyclones. While lifting the section and positioning it in place he felt pain in his left wrist. He continued to have pain but continued working at his regular duties. On May 19, 2011 he was removed from work for surgery to repair a TFCC tear in his left wrist.
While the operator was doing his hourly inspection he noticed some smoldering material on a ledge. When he was preparing to clean the material up around an exit light the light shorted and caused a small flame to come out and burn his face. The operator suffered 2nd and 3rd degree burns to his face and minor burns on his neck. He was kept in the hospital for observation.
Employee was using a punch bar to clear a blockage at the discharge chute at our RMS reversible belt. He was standing on a platform. When the material broke loose, his momentum caused him to lose his balance and fall from platform.
EE was moving miner cable when he felt pain in his back.
Pulling welder cable and heard something pop in shoulder. Did not receive report until 4-1-11.
Employee states that while walking through flypads and striking flyboard a nail became impaled in back, he was seen at ER and nail was removed by physician.
EE was fueling the truck and stepped onto containment, slipped and twisted his ankle.
EE was climbing up on our drill when he stretched his leg up on the ladder he heard and felt something in his leg pop.
Employee's hand was caught in conveyor belt.
Injured went to bolter and took a bag of rock dust off the top of the reel compartment. Bolter operator and injured made visual contact. Bolter operator saw injured get the dust; bolter operator turned back to the front and believed that injured left. Bolter operator set the tail down, pinching injured's right foot, causing fracture to great toe and second toe.
Employee states he was pulling hose out of the mud, when he felt a pop in his lower back, resulting in a muscle strain.
EE was changing the cutting edge on the JD844. While holding the edge with one hand and removing the bolt with the other, the bolt spun. It cut through his glove and cut his finger. First aid was admin. and EE was taken to the hospital for care.
Employee stepped out of office for a smoke break. When returning from her car, she fell onto the asphalt parking lot breaking her right foot. Note: Employee is not sure what caused her to fall. Possible medical condition or type of footware, unknown at this time.
Employee stated, he was shoveling the belt line, and strained his muscle, in the upper part of his leg (Right).
EE was walking to the #1 entry and walked into a roof bolt hanging from the top and hit the top of his head. The area was dry and the height was around 8-9'. The bolt hanging down was 67"" from the floor of the mine.
Employee felt abdominal pain when standing erect after lifting a section of dredge pipe.
Employee was cleaning rock and coal off top of MRS shields when a coal rib fell and struck the employee's right hand.
The employee caught his right little finger between the scoop frame and a crib block. He was attempting to remove the wooden crib block from the scoop bucket. The event resulted in a lost time injury.
EMPLOYEE WAS TRYING TO OPEN A BLOCK OF STONE WHEN HE SMASHED HIS LEFT THUMB WITH A 12LB. HAMMER
EE, electrician was unloading parts from the man trip at the end of the track rail on #3 section. EE crossed the track on the out-by end and caught his foot under the track rail. The newly installed track was not ballasted. EE hyper extended his foot and rec. a bruise. He has not RTW and does not respond to requests for communication regarding the incident.
Operator was dismounting from equipment to use the restroom. While dismounting he lost his footing and fell backward landing on his elbow. The impact caused a jarring motion and created pain in his shoulder.
While working in Phase II, employee was operating the bench loader, moving material from the face of the wall to bring a boulder down. The boulder slid, dropped and hit the front bucket of the loader, causing the rear of the loader to lift 3' to 4' off the ground. Upon landing employee felt pain to his back and neck. Employee reported incident at the end of the shift.
Employee had been having issues with his lower back which were not attributed to a particular work activity. Employee sought medical treatment and was diagnosed with degenerative disk desease in his lower back. He began missing work on 3/24/2011 to begin receiving treatment for his condition.
Injured was using come-a-longs on 3/23/11 and heard his shoulder pop. He kept working & came back to work the next day. He was throwing cable over his shoulder on 3/24/11 and it popped again. This is a re-injury to his shoulder from a previous injury that happened on 8/17/09 where he didn't start missing work until 12/2/09. He had surgery and was able to return to work on 6/15/10.
Setting rails, backed up and fell over
Employee was stepping onto a crib when he twisted his right knee resulting in a torn meniscus. Employee will be off work for surgery and therapy for an uncertain period of time.
While driving nails in the process of making belt splice, belt man felt pain in his left shoulder. He missed no work until July 1, 2011. His doctor took him off work to rest the shoulder until he could perform rotator cuff surgery.
While employee was assisting the master mechanic in repairing the bulldozer's linkage, the chain that was lifting a part, broke and struck employee in the lower left leg. Causing employee a left leg traumatic laceration.
EE mis-stepped when exiting a 544 John Deere loader. EE missed the bottom step and fell onto his backside.
Employee was loading section of steel railing into the back of service truck when his middle finger of his right gloved hand got caught between the metal and truck bumper.
EE was climbing a wooden ladder and slipped when the 3rd rung from the bottom broke. EE had a brush burn. Notified investigator 3 hrs. later. EE went to the hospital the next day. EE RTW full time 3/28/11.
Employee was pulling slack up on the miner cable. while working, a corner of a pillar rolled off striking him in the head back and leg.
Lifting large piece of stone hurt his stomach.
EE was loading a oxygen tank when his shoulder made a loud pop.
Employee was bolting on the right side of the miner in 5 South section. The bolter head plugged. As he was cleaning the bolter head, the drill steel that was in the roof fell and struck him on the right fingers. Fracture to his right ring finger and 4 stitches to his right middle finger
Employee was picking up a block of stone and felt a pain in his back.
The employee was dismounting a forklift, he slipped on a wet step and fell striking his elbow on the step.
The EE was walking in the #3 belt entry at 10 room of the C-2 Longwall Section when he rolled his right ankle on a piece of coal.
While building a stopping between 7&8 crosscut EE's left middle finger was caught between two cinder blocks. This resulted in a fracture and a laceration that required medical attention.
EE had a small piece of metal in his left eye. Metal piece was lodged there without great force as it was at the surface. Piece removed at the clinic and prescriptions given. EE lost 1 day of work. EE was wearing safety glasses at the time of the incident. Winds on the day of the incident were at or near 40mph most of the day. EE was assisting during a grinding operation.
Person Injured themselves when conduction maintenance work on a haul truck brake system. Person was lifting a brake disc and injured their back muscles. The injured person went to the ER when back pain was not controlled by over the counter pain medication. ER documented injury as strained back muscles and instructed person to take 4 days off work to recover.
Employee was walking across section, when he stepped in a hole, twisting his knee.
Employee was pulling a piece of wire from in front of the haul truck he was preparing to operate. He felt a pain in his lower back and went to the clinic where he was diagnosed with a mild back sprain. He returned to his regular job as truck driver. His condition improved until 06/06/2011 when it unexpectedly got worse and the doctor put him out of work.
Working on RB Boom and got (L) arm caught in drill pot of machine.
Strained Back, Initially viewed as a re-injury of 12/6/10 by Gallager Bassett. Employee now hired legal representation and Third Party Gallager Bassett advised new claim to be opened due to ch enge in legal status. Our first knowledge was on 05/02/11. Thus this filing. MSHA Notified by phone of this and advised this statement be entered.
Employee was involved in man bus accident that caused injury to the employee's right ankle/foot.
He was putting up wire mesh n ribs. Rock fell and landed on back of ankle, twisted leg from knee down.
Employee fell and felt a pain in his shoulder. His MRI showed a tore rotator cuff and surgery was performed. Employee's first lost work day was 5/27/2011 due to surgery.
The miner was tightening track bolts for a bulldozer Komatsu 275AX with a handle and it broke. He fell twisting his ankle.
EE was crawling and climbing removing old unused power cables at the north pump station underground. The employee strained his right knee and had surgery on August 26, 2011.
Employee was operating a DBT scoop helping to push an MRS outside. His scoop bucket climbed the back of the MRS causing his scoop to go up. He hit his head on the top causing a laceration to his ear. This did result in 8 stitches
EE was stocking out material from a series of bins. EE was returning to his truck from the loading area, as he proceeded down a catwalk, three steps and onto level ground. He tore his quadriceps muscle as he was walking down the steps.
The injured was trying to situate the cat pads into place under the miner to change them out and strained his back in the process.
Employee involved in man bus wreck 3/22/2011 but didn't request treatment until 3/25/2011 stating pain in left knee and lower back. Employee worked until 3/31 and then was took off work due to work restrictions - surgery date 4/14/2011.
Injured was picking up metal plate (app. 100lbs) felt pain in lower back.
EE was loading piece of screen onto the CM. The previous crew made the x-cut connection and the aux. fan was powered down. The slider tube was fully extended and the inby end was secured with a tube rope. The screen caught the outby end of the slider tube and it fell 2' striking the top of the EE's hardhat. The EE was conscious and transported to the hospital.
EE was sitting at his desk in a chair at the mine office. A loader operator for IMI came in the office to talk to EE. This is when EE had a seizure, loader operator said EE slumped over to his left side in his chair , and his right arm was straight out and stiff during the seizure. 911 was called, EE was taken to the hospital.
Employee was involved in manbus accident that caused injury to the employee's neck and head.
Two workers were putting together 6"" diameter yelomine pip. EE got his foot stuck in a strap, which caused him to trip, twist and fall on the ground. The resulting injury was a broken fibula in ee right leg.
Employee was totting a Flat Bar up the steps. When he made the turn at the top of the steps he felt pain in his lower back.
Workers were trying to find the problem with the power. One worker was at the substation resetting the ground fault monitor. Three other workers were standing with the IE while he was working on the ground fault monitor. IE called out and 2 co-workers pulled him off.
Injured miner was rockdusting at 55 block on 10 belt. Miner dropped rockdust hose on the floor and felt pain in his lower back while walking from the belt to the track. The miner was treated with physical therapy. No issue with training, mining equipment, methods or compliance.
While roof bolting, a pin steel came out of the chuck striking the employee in the left side jaw causing a laceration and fractures.
Employee was packing 35# sacks at the packer station when a bag blew off of the spout. When this happened, employee reached to push the stop button to stop the flow of material and his arm came in contact with the stream hydrated lime causing material to deflect up and around his safety glasses and into his right eye.
Employee stated he was changing insert on drill steel. Roof bolter operator moved the roof bolting machine forward. Right front wheel (tire) ran upon employee's right foot causing a sprain. Started missing work on 3/22/11.
Employee stated that he was letting the bolt canopy down when he caught his finger between the second stage of the canopy jack and the bolter frame.
Employee was emptying dump truck when stone inside the truck bed hung up. When it released, the truck wheels jarred the ground with employee inside the cab.
While Employee was moving a roof jack it fell striking employee on the right foot and leg.
Employee caught his L/hand - middle finger - in jaw clamps on bolter.
Pulling a 150 foot long 1-1/2 inch water hose into a crosscut and felt pain in his lower back.
Employee stated that while unplugging the scoop batteries, a wire grounded inside the plug. This caused an arc from the plug.
Employee was using a pry bar to line up a differential joint on a piece of equipment, when the pry bar slipped, causing the employee to fall to the ground landing on left wrist. Employee did not start missing time until 3/22/11.
Injured employee was lifting the miner cable up to be hung by another employee. When he said he felt a pain in his neck and shoulder.
As employee was pulling a piece of floor grating to stand on to work on a dryer unit when he felt a sharp pain in his back.
Employee shoveling under 4C belt A frame. Employee was lifting shovel over head to place material on belt when she felt/heard a popping in her shoulder.
Employee was installing top belt conveyor structure and as he was lifting the structure he started to fall and grabbed the belt conveyor rail and felt pain in his upper left arm. NOTE: Employee continued to work until 07-11-11 and started missing work as a result of his injury.
Miner was climbing off of a claw machine when he slipped in diesel oil injuring his left knee.
Employee was climbing over a conveyor belt and slipped and jumped down aprox 4' to concrete slab, resulting in a fracture to the heel.
EE was assisting in moving a section water pressure pump. He was securing a chain on a hook that was mounted on the ram of a scoop. As the ram retracted, the employees hand slipped behind the ram and was caught between the ram and the bucket frame. X-rays reveled in-line fractures to the right little finger and ring finger.
Employee had been working around plant site #12 and was walking between log washer and pipes. He slipped and fell in a sitting position on his behind/back side.
Employee riding on a bus that wrecked, causing him to hurt his back. EMPLOYEE DID NOT START LOSING TIME UNTIL 7/23/2011 DUE TO INCREASED PAIN REQUIRING EPIDURAL BLOCKS.
Employee was exiting a dozer when his right foot slipped causing him to twist his right knee. Employee did not start missing time from work until 03/24/2011.
EE was helping to install a tilt cylinder on a Komatsu loader. As he was trying to align the cylinder with the mounting bracket it was moved by the boom truck operator and he stated he was jolted, causing pain in his lower back. EE continued to work until 3-22-11.
EE reported pain in his shoulder. He said he did not have an accident but the pain persisted. He saw a Dr. and was told to return to the neurosurgeon that he had been seeing for a previous injury to the shoulder. Worker Comp. denied this claim and ee remains off work while MRI and other tests are being performed.
Employee was cutting the tip off a silicon tube when the knife slipped and stabbed himself in the right leg above the knee.
While raising the hood on a lube truck, employee felt pain in his lower back.
EE, loader/hatcher, was hatching a Sandpro truck. The truck was not washed out properly and the resin/chemical residue blew into his eyes. This cause his eyes to be irritated and burned. He was wearing proper P.P.E. during the incident. He was treated at a nearby emergency room and returned to work on 03/21/2011.
Employee was loading Hydraulic Oil on the Ride, while loading the oil his foot got stuck in the mud, when he turned he twisted his left knee.
Employee lost footing on soft berm material falling backwards. He landed on his left side with his left arm extended behind him.
Injured was operating shuttle car when he hit hole causing head to strike canopy.
Employee was shoveling belt when his shovel connected with a rock and he felt a pain in his right shoulder. Employee initially returned to work after accident until his surgery date of 4/21/2011.
Employee was building a stopping and stepped off of cinder blocks and twisted his left knee and fell when he stepped on the edge of a pallet.
EE was pushing in a shot in an open pit when the motor of his D11R Caterpillar Dozer caught fire. EE disengaged the fire suppression system and the fumes began to fill up the cab before EE could exit the dozer. EE suffered an inhalation injury with internal chemical burns and was released from the hospital 03/18/11.
Employee was raising a hinged cover on the highwall miner launch when the cover slipped out of his right hand and fell on his left hand and fingers resulting in a fracture and laceration. The highwall miner was idle preparing to change location.
EE stated he was shoveling dirt under the conveyor belt,twisted and felt a pop in his shoulder.
Employee was struck by a drill steel (12""x4-1/2"") when it fell from a Gill Rock Beetle Drill. Employee suffered collapsed lung, fractured vertabrae, ribs and collar bone.
EE was approximately 20 feet away from a mechanic that was using a grinder when the grinding wheel disintegrated and a piece flew over and cut his right wrist.
EE was carrying a 20' section of 2"" tubular steel into the shop when he turned and twisted his right knee.
Member was standing on 1' x 2' platform raising flop gate at head of reclaim belt when he lost his footing and slipped on the platform. Rain and slick conditions caused member to slip off of the top step and bump his shin and knee. Member saw a doctor where he was told to remain off work for a few days due to excessive swelling on his knee and fluid building up around the area.
While hauling supplies, he ran over a rock & was thrown into the top of the scoop, striking his right shoulder. Went to get x-rays, never missed any scheduled shifts. Worked normal duties until April 18, 2011. At this time he brought me a doctor's excuse stating he would have to be off work & get surgery on his right shoulder. Became reportable at that time.
EE stepped over the curb that's located at the west end of the Clinker Cooler at its southwest corner next to the staircase. As he stepped over the curb he felt a pop in his left knee.
Employee was shoveling on the belt line when a rock approx. 2 feet around and 12 inches thick fell from the roof and struck him on the head causing strain and pain to his neck.
Employee had exited the deck of his scoop when a piece of rock rib rolled out striking his right foot.
Member was pulling a water line with the scoop and got his right hand tangled up in the loop of the water line. The member immediately ceased to work and co-worker performed first aid on him in the nature of wrapping his hand then he transported himself to Raleigh General where he was then diagnosed with a broken hand.
Employee was being trained to operate a haul truck. The vent fan was turned on and dust was blown into employee's eyes which caused a cornea abrassion to right eye.
Employee tripped with pallet next to the shrink wrap area while trying to replace shrink wrap roll. Employee was carrying the tube that holds the shrink wrap roll and when he fell, the tube fell on his left hand, fracturing two fingers.
Employee was loading belt structure in belt sled from recovering structure from conveyor belt line. He began to feel pain in his lower back and down his left leg.
The pipe line far dredge #2 blew apart. Putting pipe back together the bank slide down (mud slide) and hit 2 employees legs.
The dozer employee operates was being welded on. Employee stood in the area while welding was being performed without the proper protective equipment on causing eye irritation.
Employee was assisting with a belt move. He was pulling on a top roller and felt a pain in his back.
Injured was shoveling the off side of the belt near the rock box when he began to experience lower back pain. He finished his shift but later went for medical evaluation and was diagnosed with a lower back strain and he was excused from work.
EE stated that while installing an eye bolt, he was holding the bolt and swung the boom of the bolter to the bolt and pushed the wrench onto the bolt but did not remove his finger. The finger tip became caught between the bolt and the wrench causing a avulsion to the finger and a fracture. The avulsion required stitches.
EE reported sweeping/lifting 20 lb bxs. Exp sudden acute pain. He went to the restroom. His RT testicle had ascended into his abdomen. He reported extreme discomfort/concern. After 30 mins his testicle descended. DR couldn't attribute to anything. Not until appt on/near 03/22/11 did the EE report a diagnosis of groin strain. Employer is still without backup & conf.
While gathering tools after working on the miner section of the rib rolled off catching back of employee's right foot causing a simple fracture of the small distal toe. Maintenance crew had moved miner to intersection and checked area prior to performing work and no hazards were noticed.
Cable was hanging from the top, something caused the cable to come lose. The cable struck the injured employee by hitting him in the chest and putting him on his back.

EE was helping install a punch plate on the screen deck and had his fingers around the plates opening as it was being pushed into place. Caught finger on cross rail, ripping open end of finger, which had to be sewn back together at hospital.
EE tripped and hit his face on the concrete floor.
Employee was fixing back board at belt drive, when he slipped, striking his left side on corner of belt drive.
Employee was hanging the roof bolt machine cable while standing on a bucket. When he stepped down he twisted his right ankle.
EE was standing approx 4 feet from scoop bucket while the forklift operator was picking up a pallet of rock dust to roll it over lathe scoop bucket. As the forklift operator picked up on the pallet it became fouled on the side of the scoop bucket and a piece broke off and struck ee above his left eye causing a laceration.
While on mantrip, face contacted roof, resulting in cut over eye and blackening of right eye.
Employee was walking up entry to examine work area and stepped into a tire rut in the mine floor causing left knee to twist.
Employee stated that he was helping to make a belt splice when he was struck by the hammer used to drive the belt nails. Co-worker was wielding the hammer and accidentally hit EE's right middle finger.
On 3/14/2011, EE reported to his supervisor that his back was hurting. He had been pulling on a knotted continuous miner cable and strained his back. He continued to work and later went to a chiropractor who referred him to an orthopedic surgeon. An MRI revealed injured discs. He started missing work on 3/31/2011, and will off at least 2 weeks depending on treatment.
Employee was standing on a 3 step stool using a 24'' pipe wrench loosening a 2'' pipe. As the pipe came loose the wrench slipped and he lost his balance having to step down off the stool quickly straining his right calf. Employee was seen at the emergency room and lost 3 days work. Returned to work 03/18/2011.
Employee was walking through water and mud to get to his water pump and stepped on a rock twisting his ankle.
The operator of water truck 498 lost control and struck a berm. The water truck flipped over. Accident still under investigation.
EE opened air lock doors and the second door closed on him.
Employee called 3/14/11 to say he wouldn't be in. Called back on 3/15/11 to say he hurt his knee at the mine on 3/12/11, but didn't know how or where. Brought doctor's excuse on 3/17/11 and said he could report back to work 3/21/11. Called back 3/21/11 and said he had a torn ligament.
The employee stated he injured his right middle finger allowing to rest on the drill pot while installing a bolt and the finger was caught between the drill pot and bolt. He is missing work.
Operator transporting stone on loader forks, when right fork broke, causing loader to rear and bounce.
He was on the top deck of a trailer that he helped load an old junk earth scraper that was attached to semi tractor trailer that was being hauled off the premises. He stepped off the trailer and turned and rolled his right ankle.
Employee tripped on bottom 2 of 5 steps exiting toilet facility.
Employee was riding in the inby end of a open ended track vehicle when the vehicle struck a track stopping device (dog)causing the vehicle to come to an abrupt stop.
Dark outside, oil containment grating was set on a walkway that normally is not placed in that area.
Putting a battery on the equipment slips and fall on a van.
On 3/12/11 employee reported to his supervisor that his eyes were irritated and that the night before when he got home after work and removed his contact lenes, there were coal fines and rock dust under his contacts. The next day the employees eyes were swollen shut so he was taken to the hospital where his eyes were flushed out and he was advised to get prescription glasses.
Member was setting bits on the left continuous miner and smashed his ring finger with a hammer which cut his finger and cause his fingernail to fall off. Member immediately ceased work and was transported by paramedics to the hospital. At Raleigh General Hospital he was told to remain off work in order to avoid the risk of infection.
The EE was operating a C.M. in the face of the #2 heading, he was in the process of setting the miner over when a piece of draw rock fell from between the installed roof bolts and struck him on his left foot and resulted in a laceration which required stitches.
Employee was operating a scoop and was bounced upward and struck his head against the scoop canopy, resulting in pain to his neck.
Injured was paying out wirerope cable from tugger winch through sheave wheel. His hand was on cable and he inadvertantly payed cable in and hand was caught in sheave wheel.
While changing lances on SSK#3 the lines were hooked up incorrectly causing a fire outside of the kiln and an overpressurization of the baghouse causing it to rupture.
Miner was in the west end of the Miracle Run Motorbarn taking off a hydraulic motor for the fan on a 15 man diesel mantrip. He stepped up on a stool to reach the hydraulic motor and felt a pop in his right knee. No issues with training, mining equipment, methods or compliance.
The scoop operator was trying to get a piece of rock off of the continuous miner by pulling/pushing it with his hands. While doing so, a piece of rock slide off of another rock and struck his right hand between the rock and the rub rail of the miner. His injury was a fracture to the little finger and ring finger.
Person was removing a chain from a belt top roller the chain sliped and hit him in the left leg just below the knee
Two scrapers were engaged in push-pull operation when lead machine hit a rock, causing it to stop before rear machine could hook-up. Rear machine then ran into back of lead machine. Operator of rear machine strained back muscles.
The employee stated that he was roof bolting the inby corner in # 9-8 crosscut. He was installing his second corner bolt. As he was directing his pinner steel up to the top a rock 11"" x 9"" x 5"" fell striking him on his right hand and wrist. He refused medical treatment at the time of the incident. He had surgery on 3/22/11 to repair a fracture of his little finger.
He was putting up a roof bolt the end of the bolt broke. He drilled another hole beside the bolt that had broke the drill steel drilled into the broke bolt, when he let the head down the drill steel hit him on the left side of his neck.
Employee was pulling on female cathead bracket on a highwall miner conveyor car. The hinge pin was rusted, so he pulled hard on the bracket lever to free the bracket. He felt pain when he jerked on the lever. Shoulder strain.
The employee was closing a crusher cover using a ""porta power"" hydraulic unit. The lines were connected from the ""porta power' to the crusher cover lines. The crusher cover hydraulic line failed at the fitting and used a whipping action. The line hit the employee in the groin/ testicle area.
On 3/10/11 the employee reported that while pulling belt with a scoop, the scoop slid over and the battery end of the scoop struck him on the leg. Employee did not miss work until 3/14/11.
Laying track tightening up bolts in fish plates pulled muscle in lower part of stomach.
Helping clear a blockage in pipe in washer plant, employee was reaching and pulling and felt a pain in his right side. Went to doctor on 3/14/11, pulled muscle in lower back.
Individual was carrying crib blocks and tossing them in a pile when the individual felt pain in the right shoulder. Individual continued to work until 4/8/11 when ee could no longer work.
As employee was investigating a leaking pipe, the step ladder he was using kicked out from beneath him. Ladder was positioned on stable ground - all four legs. Employee fell from the fouth step, glancing off underflow box and onto floor.
Employee was struck on top of hard hat while cleaning tipping valves.
Employee and others were hanging miner cable early in the shift. Employee is not certain of the time of any injury, but felt pain in his groin later in the day. Employee was diagnosed with an inguinal hernia from repetitive motion of entering/exiting the cab of his hauler and hanging cable. Employee was released for work, but began missing 3/12/2011.
Removing a rock from the Rock Box near the #3 belt drive. The rock flipped and caught his right hand between the rock and the frame. Taken to ER for x-rays and returned to work the next day. On 3-17-2011 he was informed that the x-rays showed a hairline fracture near the first joint of his right middle finger. Became medical reportable with restrictions on 3-17-2011.
Injured was loading the roof bolter in the 3-left cut on unit #2, was finished loading cable bolts and had went back to the scoop. The operator of the bolter started the machine to set over to load more supplies, injured darted back beside the machine and was struck by the machine. Injured was taken to local hospital and treated for a pelvic fracture and was released.
Member was helping re-enter hi=line cable into the #5 belt head kva. member was holding the lead out of the way for the electrician. A malfunction in the #1 box allowed that particular lead to become energized, causing the member to receive a shock.
The employee tripped over a frozen rut while walking from parking area to the garage.
Employee was checking the suction on the drill head. He placed his left hand on the drill head and the drill steel fell out of the hole striking him on his left hand.
Employee was building roof support using wood crib blocks. One of the crib blocks fell and struck him on top of his left foot.
EE was assisting the core drill operator and as the drill started he was loosening a bolt up, it spun around and mashed the little finger of his left hand.
The technician's right thumb was pinched between the hammer handle and the feeder chain flight when he struck the chain with the hammer. The result was a fractured thumb and stitches.
The employee was riding in a mantrip traveling into the mine. The mantrip hit a pothole in the road. The employee bumped his head on the roof of the trip. Upon investigation the incident appears to have avergatted an existing condition.
Employee stated that a piece of draw rock was falling and he jumped out of the way and twisted his back. The employee worked the following day but started missing on 3-10-11.
While setting a 8"" I-beam on the operators side dropped his end which resulted in fractures and lacerations to the EE on the other end.
He was lifting a breaker assembly into a panel box when he strained his right shoulder.
Employee was slewing an off-track supply car over the rail. He pinched his left ring finger between the come-a-long and jack. He fractured the tip of his finger.
Injured was pulling a beam in from the B hoist well and felt a burn in his back and numbness in his right leg. He continued to work and started missing on 7/16/11.
The employee was assisting a mechanic in blocking equipment from movement. The employee was on his knees moving a 6x6 24"" long (about 15lbs). He picked the blocking up less than a foot from the ground, and felt pain in his back. The pain, at times, went down his legs. Employee transferred himself home.
Employee slipped, tripped and twisted back, checking on pump. No complaint on 3/7/11. On 3/8/11 employee complained of back pain, was seen and treated by Dr. with pain medication. Employee was also given working restrictions.
Injured member was changing the bits on the head end of the shear when a small piece of metal from the bit or hammer broke off and became embedded in his left hand. The metal was removed on 03/14 and he received 4 sutures. He was excused from work on 03/14 & 03/15. Returned to work on 3/16 with a full release.
Employee was shoveling the belt drive area when he stepped into a hole as he was lifting a shovel full of gob to the belt. Employee twisted his knee, and felt pain when he put weight on it. Employee continued to work until 08/10/2011
While grinding metal, the operator noticed some irritation in his right eye around 4PM. He went home end of shift at 5PM and applied eye drops. Did not help. Employee went to ER early morning of 3/8/11.
He was trying to bend a 72"" bolt to install it when he strained his left shoulder.
Shuttle car cable pulled tight and at an angle. Employee tried to pick it up and hang it and a pain shot through left shoulder.
Mantrip ran into a spur and struck a parked mantrip causing low back pain to passenger.
EE was cleaning lime from the floor. He slipped on the wet lime that was due to water leaking from roof. While slipping, he grabbed the guardrail to keep from falling down and hurt his shoulder in the process. Shoulder started to swell/hurt cold compresses and visit to doctor EE now needs shoulder surgery.
Pulling CM cable and felt pain in low back
Victim stepped from lube truck to ground and slipped on muddy ground. His back began hurting. He drove himself to ER. Given a steroid injection and told to stay off work for a week. Returned to work on 3-14-11.
Employee was preparing to weld a C-clamp, he sat the clap on a table to measure it and the clamp slipped mashing his right finger.
Employee was mounting D10R dozer and slipped and fell to ground on his back.
Employee was working on the longwall recovery face and his knee started hurting on 3-5-11, but did not feel a need to have it checked out. Employee was advised to wrap and ice it. Employee was off 3-6-11, but was taken to Reliance on 3-8-11 and was placed on light duty and diagnosis with left knee sprained.
Employee was standing on a ladder hanging curtain when helper let go of ladder to pull up curtain and employee fell off.
Emp. stated he drilled hole with top steel then inserted bottom steel into the top steel. When putting the bottom steel in the chuck it stuck on the edge and popped his right hand as it went into the chuck. As a result a fracture of the right index finger was received.
Injured member was carrying a feeder jack and as he was stepping over the feeder cable his knee twisted causing him to fall to the ground. He completed his shift and the following shift. On 03/08 he went to the ER and was told by the physician that he could possibly have a Meniscus tear and scheduled an MRI and excused him from work.
Employee was looking into the balling drum-not looking where he was going, when he stepped on a 4x4, twisting his knee. He was diagnosed with a knee strain and started to miss time on 4/29/11.
Employee slipped on a 16' cable bolt in the 16H section and felt pain in left knee. He continued to work all scheduled shifts until 4/1/2011.
Employee was plastering stoppings using a spray plaster machine. He turned machine on, the relief valve was open and the plaster got into his eyes. His first day away 3/7/2011.
The injured was helping to move the section tool boxes up closer to the section. When he was pulling the tool boxes up into the scoop bucket, he strained his back.
EE works on belts. He was prying a rock out of a tail piece when the pry bar slipped causing him to jerk his back out. MRI was done by doctor but no results yet.
He was climbing over the coal pile and twisted his left knee, he went to the hospital and was diagnosed with a fracture below the left knee.
EE was pulling rock, the rock jarred the Loader he was operating and it shifted and it forced him forward, where his right lower leg hit a metal object and cut his shin.
Employee initially injured his back 9-18-08; he continued to work, still having back problems. On 3-4-11 he was working on an overcast and aggravated his back. He continued to work until 4-7-11; he then went off work pending a medical evaluation. The difference between the reporting date and the accident was pending the information from the Dr. We received it 7-11-11.
Employee was working on the #2 clean coal screen. After completing the repair work to the screen deck, Employee was climbing across the mechanism when his arm slipped out from under him. Employee landed on the screen on his back and felt pain in his neck. Employee was seen at Raleigh General Hospital on 3/6/11 and xrays were taken. He is to follow up with a specialist on 3/16/11.
Individual was lifting a ventilation tube and felt pain in the right shoulder and arm. Individual continued to work until 6/28/11, then went off work due to injury.
EE was unloading a bucket of grease from the mechanic's truck when he slipped and fell. He hurt his right knee. The doctor has not made a diagnosis yet.
EE not sure of what happened other than his eye began to bother him. EE was sent to the doctor the following Wed. due to his eye condition. SAIF has been investigating this incident and has turned this case over to the Infectious Disease Control Dept. on 4/13/11 due to the nature of the eye infection.
EE was in an area where timber was being installed when the timber was struck with an axe it caused a piece of coal to lodge in EE's right eye.
The employee stated he was walking carrying metal jack along with a co-worker when he slipped and twisted his left knee. The incident resulted in a lost time injury.
EE was laying on Dragline tub floor checking weld and twisted and felt pain in his right lower abdomen.
#5 Mine Accident: Employee pulled his shoulder lifting an outside light fixture EMPLOYEE DID NOT START LOSING TIME UNTIL 7/21/2011 DUE TO SURGERY.
Operators were winterizing an eight inch poly line. Air was blown through the pipe and was exiting a 2"" valve on the other end. An operator ensured that all air had exited the 2"" valve. An operator disconnected a 4"" quick coupler and the air that was still in the pipe knocked the operator to the ground. The medical determination was a sprained MCL. The investigation is on going.
While operating the rubbered tired mantrip the EE struck the rib and jarred his low back.
Employee was lifting crusher hammers and felt a strain in his right shoulder.
Miner was walking to equipment shop and slipped on ice and sprained his right knee.
EE was removing pipe when a valve came apart causing approx. 120 psi of water to hit him.
Parked on a hill on a mantrip. Before he exited the mantrip, another mantrip hit him.
EE was approaching steps at the dryer and lost his balance and tumbled down the steps hurting his left hand.
Worker was putting panels on the racking system and hit his head on a cross rail. EE was wearing hard hat when the accident happened.
Employee was setting up for slime outs. She stepped onto loose section of grading, this resulted her falling through to the next level. Employee received multiple contusions.
Employee was sweeping up around the bagging machine at a lower level, when he stood up he felt something snap in his lower back. After the investigation, it was determined the employee strained his lower back.
Prior to erecting scaffold a ""headache"" deck was being installed to ensure no debris could fall on the crew. As the work was being finished a chuck of debris fell from above striking the employee on his lower right trunk.
Employee was using a 16 lb hammer to straighten the bed of a coalhauler. As he swung the hammer his footing slipped causing him to miss the coalhauler bed and causing pain in his right forearm.
Employee complained of chest pains and requested supervisor to take them to the emergency room. Employee was out for observation by physicians for 3/2-3/3 and released back to full duty as of 03/07/2011.
Employee was walking between the No. 1 Refuse Vibrator and the No. 1 Clean Coal D & R Vibrator and tripped either on a make-up water line or insulating mat. He extended his right arm and hand and grabbed a support beam to keep from falling, which resulted in pain in his right upper arm.
The employee had finished drilling a hole in the roof when his steel became stuck. The employee placed his right hand between the roof and the steel to line the steel up to go in the pot, when he caught his hand between the boom and the roof.
EE called me at home Wednesday evening at about 7:30 PM to tell me his shoulder hurt and he was going to the doctor to have it checked out. After an X-ray, it was decided he had a bone cyst which was causing him pain and the doctor told him not to work for a week. I believe this is not a reportable incident since the shoulder pain is a symptom of the bone cyst.
Removing drive pulley from motor with a come-a-long when metal wedge from the pulley plate broke, striking the EE in the face.
Employee was helping unload a monorail box and as the operator of the equipment went to back up the forks went over the top of the employee feet.
EE was shoveling belt line when he pulled a muscle in his back.
EE stated that while operating a rock truck, he ran over a rough spot in the road and caused him to be jarred in the seat. EE stated that this caused him to have lower back pain.
EE was ascending stairway of crystallizer building & struck the mid rail with right knee. conservative treatment resulted in return to normal duties. subsequent complaints resulted in MRI & surgery on 8/24/11 post surgery restrictions prevented return to scheduled shift on 8/27/11
EE was cleaning the miner, while getting off the miner the EE slipped and fell causing him to injure his lower back. EE did not miss work until 3/10/11.
Employee was stepping off D11. Stepped from push arm to the ground and fell on the ground flat on his back. Employee didn't not miss work until 3/28/2011,which was when he was put on night shift he went to doctor on 3/29/11 and the doctor told him to take off two weeks.
On 3/1/11 EE was walk across the section and tripped and fell causing him to dislocate his shoulder. EE continued to work until 9/13/11 when he elected to have surgery on his shoulder.
While dragging a ventilation tube around a corner he rolled his left ankle and fell to the mine floor.
Contractor was told to plaster stoppings but had left his plastering gloves on the mantrip, which had already returned outside, so his boss informed him to plaster the stoppings with metacarpal gloves instead. Contractor later went to Raleigh General seeking medical attention, where he was told he had sustained first and second degree chemical burns to his hands and arms.
Employee was walking from #6 to #7 entry to get the scoop when a piece of draw rock fell between the rib and roof bolt striking him. It knocked him down and landed on his foot.
Contractor was performing normal work on 3rd floor of 400 building and allegedly hit his head on a low hanging duct.
Employee was bolting top in the number 3 entry when a piece of rock (slick siding) fell from the rib and hit him in the head, neck, right shoulder, rib, and hip areas. X-rays were negative. He had bruising in the areas listed. The piece of rock was 10'L X 3'W x 0-12"" thick.
EE was loading pallets on a track flat car and strained his lower back.
EE was throwing a crib and smashed his finger.
Injured employee was removing the tongue of the tow hitch from the tow vehicle. The tongue eye was on a bind and when it came loose, the tongue fell to the ground unexpectedly and struck the injured employee on the left foot.
While employee was cutting a tree with a chain saw, a dead limb fell from the tree, striking him in the back. He lost control of the chain saw and it struck him in the left leg, causing 52 stitches. He was transported to the Hazard ARH where he was treated and released.
He was driving a supply motor through a set of air lock doors and caught his left foot against the door. Fractured ankle - started missing work 3-2-2011.
While operating dozer the seat broke and bottomed out. EE reported this on 3/3/11 when he told his foreman that his back was hurting. Attached letter protesting this alledged injury. EE began missing work on 3/14/11.
Was going to shut machines off slipped and fell shoving heal of hand into ground. EE cont'd. to work then noticed hand was starting to swell and the ring and pinky fingers were numb. The Dr. stated to be off rest of the day and to wear a splint for 7 days.
Hoisthouse overhead door was closing & employee walked right under the door as it was closing, striking him in the hardhat.He reported to the medics.We took him in for x-rays,which were negative.He was issued a full release. The next morning 3/2/11(a scheduled shift for him)he felt he needed to see a chiroprator.The chiroprator issued restrictions.
Employee was moving curtain to level footwall to start brattice and stepped on a nail in an old pallet.
Employee was turning and twisting wheel at bottom of #1 Chemical bin and felt a pull when exerting turning the wheel.
A rock came dislodged and hit EE in the leg. The doctor took x-rays and found no problems. EE only missed one day and is having no further problems.
Employee was unstopping a rock crusher when he went to move a rock, and his foot fell through the grizzley, he went to catch himself and hurt his left hand.
Caught reinforcement while drilling causing EE's wrist to twist.
It was close to getting dark outside, time was a factor, employee wasn't paying attention to what he was doing. Damage is contusion to 3 fingers, stitches required in 1 finger.
Employee first reported injury on 3/31/11. Stated he had used hammer on 3/21/11 and later in the day felt pain in right shoulder. Thought he would improve, but felt it was still sore. Saw dr on 4/1/11, dr stated it was a right shoulder strain, gave him an injection and put him on light duty.
An SXEW employee was unloading a pump off of the warehouse pickup with a forklift as a warehouse employee spotted from the side of the pickup. While pulling the pallet, the pump got hung up and the second employee tried to prevent the pump from tipping, got his arm caught between the pump and truck bed, fracturing his wrist. ONGOING
Process Mechanic was loosening liner bolts when Mill was inched clockwise. His hand was caught between the handrail and impact wrench, causing a contusion. Employee worked five days and then called in that the injury was hurting too badly to work on the sixth day.
Employee pulled a muscle on his right arm while opening a train hatchet.
Employee was raking ash, the ash rake slipped off the large piece of ash and the employee lost his footing. Employee fell to the floor and landed on his left wrist. Left wrist fractured.
EE was breaking bead on right front tire of 40 ton articulated haul truck, when ram broke loose, striking EE on the left side of the face.
Employee was riding in shift change bus when it hit two large holes in the roadway. The injured employee was sitting near the rear of the bus. The incident caused him to hit his head on the ceiling and be thrown to the floor causing pain in his lower back and an abrasion to his shin. Work restrictions were issued.
Was stepping down from the overhead belt onto the screen deck, when slipped and fell over the side of screen deck onto the gearbox, hitting left shoulder. ER evaluation determined separated shoulder and referred to orthopedic for further evaluation.
While positioning a cutting edge prior to installation, cutting edge slipped onto employee's finger.
Injured was loading trucks from a stockpile with a 980H loader when a large piece of material rolled down and struck the loader arms jarring the machine. Injured initially requested medical attention, but was released with no restrictions. Injured subsequently was diagnosed with a hernia, underwent surgery on 5/21, and was unable to return for scheduled work on 5/23.
On 3/28/11 at 12:15 am the 2C big bag operator was stepping down in to the loading room from the operator station when he felt a pain in his right knee. The step is about 8-9 inches.
Employee was stepping off locomotive, lost his footing, rolled his ankle and slid down an embankment.
After our investigation, it has been determined that the injury that the employee received while shutting the van door will be claimed as a reportable injury.
EE stepped down onto the rubber dredge hose which was 2 feet lower than steel platform, when a 500 lb steel sheave & block slipped off a steel platform & struck his left ankle with a glancing blow, causing a deep abrasion & fracture. The abrasion required 3 layers of stitches.
Employee fell against an entrance door when he tripped on the last step of a stairway.
Drill crew was tripping drill rod when the sling hook came out of the pin end of the rod causing it to land on the drill table. As the rod fell to the deck, the sling hook came off and swung towards a drill helper. The helper put his arm up to block the sling hook, resulting in a fracture injury to his forearm.
Employee was boarding haul truck and felt a pop in right wrist. This was a questionable case and it was determined by our Occ. Doctor on 7/12/10 to be work related.
EE had been working on the 16G Cat Grader, when he was finished and was climbing down the ladder he slipped off of the steps and landed on his left shoulder.
EE injured right shoulder on 3/25/11 swinging a hammer was evaluated by doctor and returned to full duty. Employee experiencing pain in right shoulder and was re-evaluated on 5/23/11. Employee received MRI and referred to orthopedic specialist. Evaluated by specialist on 6/9/11. Employee received surgical repair to right shoulder on 7/5/11 and his first missed shift was 7/8/11.
EE was attempting to polish a shaft on a 3/4 horsepower motor with sandpaper when the shaft grabbed the sandpaper and his right glove twisting it off his hand. EE suffered a broken finger. He is not restricted and has not lost time from this event. These events took place on 3/25/11, but company was not made aware of this until 4/12/11
IE and two men were removing the off side planetary from the joy miner. They were using a set of come-a-longs and two ply bars. While laying on his side, EEs' ply bar slipped, causing his legs to move inward-the planetary fell out and onto his right foot-mashing btw planetary and cat track. No broken bones.
Employee was putting up an outside bolt and running head when brow fell out and hit his left lower leg.
EE was in front of the miner crawling to the next cut, thought he was ahead of the miner far enough however he never quit tramming the miner and rolled the bull wheel over his right foot with the miner bit penetrating his ankle.
Employee had finished setting bits on the shearer when he stepped over a wind row of coal and twisted his left knee.
EE was lifting a screen which weighed approx. 50-60 lbs. up into the back of a pickup truck. As he was lifting the screen he felt his elbow tear. After about three days he noticed a significant amount of bruising in and around his right elbow. EE went to see his doctor Monday March 28th, and they set up an MRI and a follow up appt. for Friday April 1.
Employee slipped, causing him to fall backwards and catch himself with his right hand. Fractured right hand in 3 places.
Employee was assisting with a maintenance task of re-assembling an auger when he lifted a blade and felt a discomfort in his abdomen that did not go away. He was sent for medical evaluation and has been diagnosed with an inguinal hernia on his left side. He has restrictions that can be met as of now, and is being scheduled for surgical repair.
Employee sustained a right bicep tear while attempting to move an 8 ft x 4 ft x 1/4"" plate. The employee continued to work, but following an MRI on 04/01/2011, it was determined that he would have to have surgery. The first lost work day was 04/04/2011.
Employee was in #6 entry behind his roof bolter when the miner pulled through the wing curtain and struck the employee in the right buttock.
While pulling on a safety cone he didn't realize that it was frozen to the ground and pulled too hard. Heard a popping in his hip.
EE and co-worker were installing an elevator plate back on to the stand when the EE felt a strain/pain in his lower back. EE was taken to the clinic to be checked and taken off work.
EE arrived at work 45 minutes early. He was walking into the scale office when he stepped on some ice on the north side of the building. EE slipped and fell landing on his right arm.
Employee was removing a piece of brattice cloth from the heavy media vessel when he felt a sharp pain down his right leg and in his back.

During an MSHA Inspection the inspector wanted to inspect the berms above the highwall, foot traffic is usually not allowed in this area. Our EE offered to drive,the Insp.wanted to walk. Crusher operator slipped on slope fell and struck the back of his head on the frozen ground. Resulted in a concussion, blood on brain and 1.5 days in the hospital.
EE was clearing rock from transfer chute when he twisted his knee. EE never missed work until 5/2/11.
On 3/21/2011 employee was pulling on a piece of belt when he strained his left shoulder. Employee became restricted on 5/16/2011 due to this injury.
Employee was placing a bag of garbage into the dumpster when he felt a pop and a pain in his right shoulder. He was treated as first aid. Medical treatment occurred on 4/5/2011.
Employee was welding on excavator bucket supported by three pieces of wooden cribbing. Bucket was attached to excavator. Two of the three pieces of cribbing split causing the bucket to suddenly drop. Employee was struck in the knee by a piece of cribbing.
While working at the main plant in the morning EE and another EE were changing screens. EE had one end of the screen and another EE had the other end. One EE's end slipped and fell on his right foot & went through his boot punctured his foot. On date of injury EE didn't want to go to the clinic & get it checked out.
EE reported on 03/21/2011 that on 03/18/2011 he was conducting routine work when he felt a ""pop"" in his left knee. Employer is ""Red Lining"" this case due to late report. EE had surgery on his left knee on 07/06/2011. Employer contends that this is not a work related injury.
Employee was walking down gravel ramp when he fell to the ground.
Employees were taking a pipe apart and working on a winch when the mechanic came around the corner to communicate to the employee inside the pilot house of the dredge, when he stepped into the pipe access.
Employee was connecting steel on the west stairs approximately 16' above the ground while tied off 100%. He lost his balance and fell, and in the process of the lanyard arresting his fall, he reached out with his left arm to grab the steel and felt something pull in his left shoulder.
Employee was installing roof bolts, he bent one and was trying to get it back out of the hole. He was using the head to pull it back out of the hole, he hit the wrong lever and spun the bolt, twisting his right wrist. And after xrays was read, he has a fracture in his wrist.
Employee was shoveling belt line when another employee put a rock on the belt and it flew off and hit EE on the forehead.
Employee was carrying a 5 ton chain hoist (approximately 50 pounds)downstairs. He was not using the handrail and was stepping in a sideways motion to ""baby"" his left knee that was injured previously. Initial doctor visit and x -ray on 3/17/11. Follow up and referral to second doctor on 3/28/11. MRI results on 4/12/11 of a tear in the acl.
EE was moving and replacing the impeller shoes. EE felt a pain in his lower back. EE was sent to doctor who restricted him for 7 days, but he has not return yet.
EE was going through double doors in belt line slipped and fell on his left knee.

Employee was picking up cones around edge of shot muck piles, fell, resulting in a twisted right knee. This was a medical treatment until 5/31/11 when she had surgery. This was not reportable until 5/31/11
Injured employee was getting off maintenance truck and his foot got caught on a burning torch hose, causing him to fall on his back.
The employee stated that he had his left foot hanging out of the mantrip and clipped his foot against a rib.
Employee got off mantrip to close airlock doors, when the employee turned around the mantrip was upon him. Pinning the employees left leg between the mantrip and the metal air lock doors. This put pressure on the employees left leg and knee.
EE was picking up a piece of steel in the yard before he lifted on the canopy he felt a pulling sensation in his right groin area. He was diagnosed with a hernia and had surgery on 6/16/2011.
EE was cutting plastic tubing with a razor knife, the blade cut through the tubing and contacted the back part of the ee's left thumb, cutting down to the tendon. The ee required surgery to repair the tendon.
Bolter operator was straightening a roof bolt and strained shoulder.
EE slipped while performing routine duties. In an effort to break his fall, EE grabbed onto a nearby structure. EE started losing work days on 09/14/2011.
Employee was in the stone tunnel when he grabbed a steering roller on the steering idler. His sweatshirt sleeve then became caught between conveyor belt and roll, pulling his hand in. He pulled his hand out of the pinch point. Off-Site Medical-Rx Drugs, Bone Fracture
The individual was using a pry bar to align a head light on the Fletcher roof bolting machine. The bar slipped and caused him to strike his hand against the frame of the machine. This resulted in a fracture of his right ring finger.
Coming down the stairs at stilt shed, lost footing fell down and twisted ankle.
Employee reported to his supervisor approximately 1 1/2 hours after his shift ended that his back was hurting and wasn't sure what he did to hurt it. He went to the doctor and reported that he was having back spasms.
The victim had his hand crushed when the supervisor mistakenly thought he was signaled to raise the drill pipe. When victim notified the driller to stop the accent, the sudden deceleration caused the pipe to fall out of pipe sling coming down and crushing the victim's hand between pipe and the hog trough.
Injured was attempting to throw a rock under the catts of the miner, lost grip and rock smashed middle finger on left hand. Injured was taken to hospital and x-rays revealed a fracture of the finger.
Employee was injured on 3/8/11 when he was struck in the left knee by the end of a 20' piece of 6 inch aqua pipe that was being pulled around a corner. Employee continued working until 6/15/2011, at which time he had surgery on his knee.
While bolting the back, miners drill bit came off the end of his steel and landed in a puddle beyond the last row of bolts. The miner went past the last row of bolts to retrieve the drill bit and was hit with rock that fell out of the back.
Drove mucker bucket down to the work area. Miner ran over a big rock by first muck bay. After he parked and got off mucker the miner bent down to pick up water hose he felt a shooting pain down his lower back then down his left butt cheek.
EE told a supervisor he felt a pull in groin. The details didn't appear to be something that could be work related so the supervisor told him he could leave. The ee told the supervisor later that he had pulled something in his groin while moving a screen. Information obtained from the doctor indicates this could be a recordable inj due to conflicting MD information.
Employee slipped and fell on ice between shop and steambay dislocating knee cap.
EE lifted an oxygen cylinder to put on a cart when he felt pain in his lower back. He had surgery on July 29, 2011.
