An unintentional roof fall occured on HG#5 #1 entry break 3. The fall measured 18'w x 20'l x 8'h.

A roof fall meas. approx. 18' wide X 15' long X 6' high was discovered in the #4 intake entry of the west mains area between survey station # 110 and 118. The intake was routed around the fall. The fall will not be cleaned up. There were no injuries involved.

An unplanned roof fall occurred outby the #1 section in the #8 entry belt entry inby spad #6716. There were no injuries or equipment damage due to the fall. The fall will be cleaned up according to the fall clean up procedure which is posted at all approaches to the fall area. The fall dimensions are 17 1/2 ft W X 50ft L X 6 1/2ft H.

A precautionary immediate report was made because of the potential for the inundation of water into the pit. A creek adjacent to the pit overflowed during a significant rain event resulting in water overflowing the creek and going into the pit. There were no injuries and no health concerns.

Heard a noise coming from the service elevator. After investigation it was found that a bearing to the governor rope was bad. Elevator was taken out of service at 12:55pm and put back in service at 3:27pm.

An unintentional roof fall was discovered by an examiner in the 8 Haulage Bypass #3 entry at the #12 crosscut. The fall measured approximately 30'L x 20'W x6'H. This fall occurred in a neutral entry and no ventilation or travelways were affected.

A roof fall has occurred in the return bleeder panel in the #2 entry at brk.11. The size of the fall is approx. 19'W x 50'L x 6"" thick. No ventilation was affected, no equipment affected. The area has been timbered and dangered off.

While hoisting rock, the 3rd shift hoist operator had the brakes set and lock up on the #1 hoist. After troubleshooting, maintenance personnel determined that a faulty output signal was affecting the hydraulic system. This output card was replaced and the hoist was put back into service. The mine maintained two means of escape with the two other operating personnel hoists.

Roof fall occurred in 1 Panel Right in the Campbells Creek Seam, 4000 feet from active section in the neutral. Starting in #3 entry at spad #41 thru crosscut at spad #43 in #4 entry thru crosscut to #5 entry toward spad #39. Fall was 3 to 5 feet thick.

The cont. miner on the 004-0 (""U"") section accidentally cut into an active gas well in the face of the No. 4 entry. The gas well had not been plotted on the mine map at the mine. No gas escaped into the active workings. The power on the section was cut off and the entire mine was evacuated and the underground power was cut off.

A leak in the J tube of the cooling line allowed a section of fire wrap to ignite. The small flame was immediately extinguished. The flame reappeared and was successfully extinguished with water. The flame was contained to within the confines of the motor compartment. No individuals were injured. No area of the mine was damaged. Total time did not exceed 10 - 15 minutes.

There has been a fall located 1 break inby survey station No. 776 in the No.4 to No.5 Entry. This fall did not impede travel. The fall has been cleaned up in accordance to the clean up plan.

Pellet plant freight elevator was stuck between floors with a man inside it. The elevator was repaired and the man was safely vacated the elevator with no injuries.

An unplanned roof fall occurred on Crosscut 93 Entry 2 towards Entry 3 35' Main West 2 Return. The roof fall measured approximately 55' in length, 17' in width, and 5' of thickness.

An unintentional roof fall occurred on the #3 Southwest Panel at #1 break in the #1 entry approximately 20 ft outby spad #1129. The fall measured approximately 80 ft x 20 ft x 7 ft high. Area was timbered and dangered off. This fall was discovered during a weekly air way examination.

Employee detected roof fall that exceeded the bolt length anchor points. Mine Manager, phoned the Rolla office with no response. He then notified the One Call line to report this incident. MSHA investigated the accident on 2/24/2011.

Elevator in #6 could not be operated due to a fuse blown in the substation during an electrical storm and a hydraulic hose failed on the back-up generator. The fuse was changed and the hydraulic hose replaced an the elevator was back in service at 11:40 a.m.

A roof fall occurred in heading #25 between 25-2 & 25-6 blocks. The fall was approximately 450' long & spanned the width of the 60' heading. The area had been previously supported by 14', grade 75 mechanically anchored roof bolts installed on a 5 x 5 pattern.

Roof fall measuring 25' long, 20' wide, and 8' thick was discovered in the #4 entry at survey mark 66128 approximately 2000' from the working face. The area was developed 2/4/11 with 6' torque tension roof bolts as the primary support. Fall did not impede travel, nor affect ventilation. No injuries occurred as a result of the fall.

A roof fall was found in the #4 track entry, between break 17 & 18 the size of the fall was approx. 18' x 22' x 6'. The primary escapeway has been re-routed to #3 belt entry, the effected area will be cleaned and bolted with 10' & 12' rope bolts and 4' glue bolts.

A roof fall was discovered in the intersection at spad 1731. The area was 16'X 20'long X 5'high. The fall did not affect miners nor equipment. The travelway was re-routed and supplemental roof support in the form of cribs and timbers installed.

Employee was tramming a D11N dozer out of a shot and when he got to the haulroad he noticed that the hydraulic gauge was showing hot. He stopped the dozer and trammed it backward to a wide spot (50 to 60 feet) to keep from blocking the haulroad. When he stopped, fire shot up beside both doors. The right side fire lesson and he exited the dozer. No injuries occurred.

The #1 elevator was out of service for a period of time exceeding 15 minutes. One individual was on the elevator at the time. The elevator was down from 3:07 PM until the individual was removed @ 3:35 PM. The governor switch was re-set and the clip for the switch was tightened. All necessary government agencies were notified according to law.

ARSI employee leaned up against the inner service elevator door and caused the K3 elevator to stop while it was traveling from the 4th floor. Executive Elevator Co was called. Employee got out at 1:30 AM. Signage inside the elevator reads ""Do not attempt to stop elevator by opening car gates. Wait until elevator comes to a complete stop before opening car gates"".

#3 Production hoist tripped on a drive convertor fault. The electricians were called from underground to surface to reset the fault. There were still two means of egress to surface for personnel underground.

C.M cutting #2 entry, they backed miner out to clean scrubber in #2 entry x-c9, while cleaning scrubber the top edges of along rib line started to dribble, men vacated the area and used the miner remote started backing miner out when middle of intersection fell on miner then gradually the entire intersection fell in on most of miner. Fall area 20' x 20' x 8 - 10' thick

The #1 elevator was out of service for a period of time exceeding 30 minutes. The PIT switch was reset and tested and the car returned to service @ 3:55 PM. No individuals were on the elevator at the time. All escapeways were re-routed.

A roof fall occurred in the 1st West Sub-main, Spad 10+50, entry #9. The fall measured 18' wide X 6' high X 30' long, approximately.

Occurred during the week of 2/10/11 thru 2/17/11. Roof fall was found on the weekly examination by EE and MSHA inspector. High 12ft X Width 20ft X Long 45ft at spad No.4t80 in the main west No. 1 entry. Return air course.

Miner oper. had taken a cut of coal out of face of No.3 hdg. that was approx 15' in depth. After the coal had been removed, a section of mine roof in unsupported area of newly mined cut fell. Size of section of roof that fell was approx 14' wide, 12' long, 0-2.5' thick. The material was sandstone. The ignition occurred when miner oper. started to cut the material. Put out 3-5 mins.

An unintentional roof fall occurred in the old worked out 1st East panel off the 1st South Sub-main. The fall was at station 14+00 in the #8 entry. The fall measured 20 feet x 20 feet x 6+ feet high. The fall did not affect ventilation, passage of men or cause personal injury.

There has been an unintentional roof fall in the #2 entry, in the Main West Return, @xc-26. Approximately 25' long, 17' wide, 3' high.

IGNITION: A frictional ignition occurred while cutting the top at Shield 19 on #2 Longwall. It burned approximately 1 to 2 seconds and was put out with the drum water sprays. No injuries were incurred.

A roof fall measured 12'long x 16' wide x 6' thick occurred in the #6 entry in the escapeway between cuts 188 & 189, 3600' outby the face area. The area was bolted with 6' tension rebar bolts and supplemented with 12' cable bolts on 16' centers. This area is under stress from over mining. Some water was present in the roof.

A roof fall was discovered in the 4N panel at spad #16+10,#4 entry, xc-23. The approximate size of the fall was length-22ft, width-18ft and height-6-8ft.

While traveling the B-Mains Bleeder Return, a fall was discovered on the #24 Block, #10 Entry of the 4-Mains Section. The fall area was 3900' outby the face and was bolted with 6' Super Twist bolts. No people or equipment was affected.

The section foreman noticed that the roof began working approx. 55' inby station #2336 located 1 1/2 cross cut outby the face of #2 entry. Additional roof support was installed, however, it caved on a battery scoop for approx 60' and 12' high. Additional support was added and accesses to the fall were dangered off. Work is being done according to the approved plan.

A roof fall was found in the Double Submains in the #1 entry at break 11, the size of the fall was approx. 20'x20'x6'. No ventilation or equipment effected. Additional roof support of timbers and cribs have been placed in affected area.

A roof fall occurred in the #9 seam, 1st West Sub-Main at crosscut #8 in between the #9 and #10 entries. The fall measured approximately 50'x17'x8'. The area was supported with 6' tension rebar roof bolts. An undetected slip was determined to be the cause of the fall.

The emergency escape hoist became inoperable during an inspection by MSHA. The braking system on the hoist became engaged during normal operations. The lilly overspeed was adjusted and the hoist returned to normal operations at 6:00 PM.

While traveling the B-Mains Bleeder Return, a fall was discovered on #32 Block, #8 Entry of the 4-Mains Section. The fall was 3200' outby the face area and was bolted with 6' Super Twist bolts. No people or equipment was affected. This entry was one of the two return air courses.

Employee was checking a discharge line for a pump and discovered the fall in the #6 return entry that was approximately 70'L X 20""W X 7""H. All entrances to fall area have been breakered off.

A roof fall occurred on this date. The fall was located on 3-North Mains at break #70 in the #6 entry. The fall measured 25' by 20' and was 10' in thickness. The area was previously supported with 5' Hy-Tech bolts. The area has been dangered off with cribs set in the area. The primary escapeway was rerouted. There was no persons affected.

A roof fall measuring 20'long, 18'wide, and 8'thick was discovered in the #5 entry outby spad 66202. Fall was 600' from the working face and did not affect ventilation or the passage of men. Area was developed 2/11/11 with 6' torque tension roof bolts as primary support. No injuries were reported as a result of the fall.

An unintentional roof fall has occurred at the Main East Xc-10 travelway. The fall was approximately 20 feet in length and 25 feet in width and 9 to 10 feet high approximately. A action plan was instituted to rehabilitate the affected area.

A roof fall occurred in the South East Main at cross cut 31, # 6 entry. The fall measured approximately 50 feet long by 18 feet in width and the thickness is 8 - 10 feet. The area was supported with 6 foot fully grouted roof bolts.

During weekly examinations, an unintentional roof fall was found 3 breaks outby the face area of the old 003 section. Not an active section. The fall was 1 break inby spad #8927. The fall was approx 50' long x 18' wide. Fall was in #4 entry. Height was undeterminable. Area was timbered/dangered off. No injuries or equipment involved. Will not be cleaned up.

The mine roof on the No.3 section (MMU-009) fell above the anchorage point in the No.4 entry inby spad 28065 for approximately 90 feet and in the No. 3 entry inby spad 28064 for approximately 70 feet.

A Continuation of a roof fall that was reported on 7/16/2010 was found during a weekly examination. The fall was found in our M4 Mains, X-Cut 72, #3 entry and was the intersection only. It measured 20' X 20' X 15' high. The continuation did not affect passage of miners, affect ventilation, or cause injury to miners.

A roof fall measuring 36'long, 19'wide, and 12'thick was discovered in the #4 entry 6 belt inby spad 65935. The fall was 650' from the working section and was developed Jan 2011 with 6' and 8' torque tension roof bolts as support. The fall did not impede ventilation or the passage of men. No injuries were reported as a result of the fall.

At 2:18pm the service elevator at the bottom landing went down as it was being attempted to be called to the top landing. The service elevator breaks would not release to allow the elevator to come to the top landing. Replaced two relays and the elevator was put back in service at 6:45pm.

IGNITION: The miner was cutting bottom rock in the No. 1 Face on No. 7 Section when a frictional ignition occurred on the curtain line side on left of the Miner Head. Approximately 2 fire extinguishers and the water sprays put the ignition out. No injuries were incurred.

A reportable roof fall was found in E2 Headgate, entry 1, at crosscut 60. The fall was approximately 16' wide, 35' long, and up to 9' high.

An unintentional roof fall occurred on the 2nd West,station 63+73 in entry #4. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 30 feet x 20 feet x 7 to 8 feet high. The intersection had been bolted with 6 foot fully grouted roof bolts, approximate date of development was 5/20/09.

Pleasant Grove elevator was stuck at the bottom of the shaft and would not respond when activated. The DC contactor went out on fault. DC Contactor replaced to return to operational capacity.

Flint Ridge Mine #2 has experienced a non-injury unintentional roof fall out by the 002 section in #5 entry (neutral) at cross-cut #34 on 3 Main. The dimensions are approximately 7 feet thick, 35 feet long and 20-30 feet wide. This area was initially supported with 60 inch fully grouted resin rods.

A roof fall occurred from the track entry to the cross cut #1 & #2 below spad #424. The fall measured 180' long X 18' wide X 5' thick. Persons nor ventilation were effected but the main line belt was covered. This area will be rehab.

An unintentional roof fall occurred at 6 1/2 x-cut in 4 South. The fall measured 16ft wide by 20 ft long by 12 ft High. No injury or equipment damage as a result of this incident.

This incident was called into the MSHA 24 hour hotline at 12:20am on 2/13/2011. There was an unplanned ignition of lubricant in the Kiln #1 Bull Gear housing. The fire was first noted at 10:15pm. It was extinguished at 10:35pm but continued to reflash until 10:50pm. MSHA inspector of the Rolla, MO field office was onsite conducting the investigation on 2/16/2011

A roof fall occurred in the #8 entry of the 2nd main west old #1 unit. No injuries or disruptions to ventilations. The fall was approx 55' long X 19' wide X 8' high. The fall was discovered at approx 1:33pm between x-cut 79 and 80 inby spad 5 + 46.

An unintentional roof fall occurred in the No. 5 entry, one break inby spad No. 7797 on the no. 1 section. The fall measured 35' long, 18' wide and 6' high. The fall did not affect the air flow or the travel of men.

Roof fall occurred on the 6B miner section. Fall was located at 43 wall between the track and return in the crosscut on the track side of the wall. Dimensions measured 40'long X 15'wide X 15'high.

A roof fall has occurred in the left return at spad #126. The fall measured 25' long by 20' wide by 5' thick. Persons, equipment, nor ventilation were involved. This area will not be rehabilitated.

During the preshift of the 001-0 MMU, T-Section, at approximately 5:50 a.m., the preshift examiner encountered an explosive mixture of methane in the range of 5% in the #1 entry and 3.8% in the #2 entry of the pillar line.

A roof fall was found in the old Double Submains at break 11 in the #2 entry. The size of the fall was approx. 50'L x 20'W x 4'. The area has been dangered off and additional roof support of cribs and timbers will be added.

The employee was pushing coal off of the tepee and pushing back to the east. When the employee was making a push the coal gave away beneath the left side of the dozer causing it to slide down into a void. There was no injury or equipment damage.

There was a fire at the Tyrone Hydromet Filter Clay Press, a mayday was called at 3:54 AM. Safety Pro notified MSHA at 4:17 AM of the fire at the Tyrone Hydomret. The fire was called out at 5:55 AM and Safety Pro called MSHA at 6:00 AM to notify MSHA that the fire had been called out.

D 11 N Dozer fire started under floor boards of the cab. Our men put the fire out in approx. 7-8 minutes. Fire re-ignited and the fire dept. had arrived and got the fire out in approx. 30-40 minutes.

At 12:20 PM on 2/8/11, a roof fall was found in an intersection in 4 west mains at survey station 3138. The fall was 25' long by 20' wide by 8' high. No injuries or equipment damage resulted and the fall did not affect ventilation or travel. The fall will not be cleaned up.

An unplanned roof fall occurred in 4A 3 Right Panel in #2 entry 140 inby spad 1278. Fall was approximately 17' wide X 6-14' height X 50' length. No injuries, entrapment or equipment involved. No escapeway, ventilation, or travel way affected. Area developed January 2011.

An unplanned roof fall occurred in 4A 3 Right Panel in #6 entry at spad #1293. Fall was approximately 17' wide X 14-16' Height X 55' in length. No injuries, entrapment or equipment affected. No ventilation, escapeways or travel ways involved. Area was developed in January 2011.

An unintentional roof occurred at break 104 in the #4 entry (belt). Size of fall was estimated to be 12'L x 7'W x 3'-31/2'Thick. No injuries, ventilation was not interrupted, belt line was caught. Fall will be cleaned up. The roof had been supported with 6' fully grouted resin bolts

An unintentional roof fall occurred in the 8Q longwall recovery face between the #4 and #5 monkey butts. The fall is approximately 60' L x 18' W x 12' H.

On 2/4/11 an unintentional roof fall occurred. The fall occurred on Old #8 belt right return #8 entry, break # 11 &12 spad #3180. This is a continuation of an old fall reported on 10/16/10. No injuries occured from this fall. Length 100' X 19' width x thickness. Above anchorage. Support 4' fully grouted bolts.

Roof fall was discovered in return 3200' from working face and measured 50' long, 18.5' wide, 12'thick. Fall was located in Southwest return panel and was developed in 2009, 4' glue bolts were the primary support. Ventilation and passage of men not affected. Fall area will be barricaded and dangered off.

The site had an unplanned ignition of coal dust in the #2 Kiln ball mill fuel system at 2:07 pm on Feb. 3rd. The ignition occurred when the ball mill fan was started and reached 45% output. No injuries resulted. Minor damage occurred to a pressure relief vent. The event was reported to MSHA at 2:15 pm.

Due to an area electrical outage, power was lost to the mine. After trouble-shooting to re-set power, a problem existed with the man cage. Mine evacuated. No problems; no injuries. Power restored to man cage at 10:40pm.

A roof fall was discovered in the #3 entry of West Mains @ Spad # 196. The fall measured 40'long by 10' high by 16'wide. The area was bolted to plan and also had been supported with additional roof support consisting of dywi-dag sings, V-beams and rock props spaced on 4-ft centers. No injuries or equipment damage. Immediate notification was made after the fall was discovered

An unplanned roof fall in #2 & #3 room on 2nd south butt 80' inby spad #4846. The fall was about 50' long x 20' wide in both rooms & crosscut 3-2 room. the highest part of the fall is 12'-14' high. The rooms were bolted with 42"" fully grouted resin bolts until we encountered sub normal roof in the #2 room. We rebolted outby the sub normal roof with 6'-8' install #2 bolts.

A roof fall was discovered on the #2 section, #8 entry, 1 crosscut out by spad #464. The fall measures approx. 80 ft. long by 20 ft. wide by 6 ft. high. No injuries resulted from this fall.

During a weekly examination by ee of the right return it was discovered that a roof fall had occurred on Panel 6 at spad #1525. Stopping were installed and removed to allow the return to be safely traveled in its entirety. All accessible entries leading into the fall area have been supported and dangered off.

The East Finley Elevator was down due to the door hold button Malfunction. A wire came off and the unit would not function properly. No damage to the elevator and no one was on it at the time of incident.

Roof fall discovered in return 3900' from working section measuring 80'-150' long, 160' wide, and unknown thickness. Fall was located between #7 and #8 entry, the area was developed 1/20/09 with 4' glue bolts as primary support. Fall was immediately reported and investigated by MSHA.

EE was getting back in his shuttle car on #8 unit after a lunch break and his feet were not in the cab when a scoop operator (scoop) hit the opposite side of the car shifting it in the process. EE left foot got wedged between the car and a rock injuring his left foot. He did not begin missing work until the following week (3/9/2011).

While adjusting a part that was put in the back of a truck, employee stepped back and into a track, causing him to fall onto his left hand. Required stitches.

While pulling the miner cable and water line away from the continuous miner a rock measuring approximately 62""w x 46""L x 6"" thick rolled off of the side of the machine and landed on the miner operator's foot. The continuous miner was de-energized at the time.

After washing off the oil accumulation on his equipment, at the wash bay. Employee was walking towards washer wand storage bracket. He slipped on ice and landed on his left knee.

Employee was working with his crew at the bottom of the pre-heater tower north end area. He was cleaning build-up material. Cemex employee was driving a Bobcat towards him & his crew to fill bucket with material. Once bucket was full, the operator leveled the bucket flush with the ground. Employee's foot caught under the bucket. He screamed to get bucket lifted. Taken to doctor.

The scraper was traveling on a scraper road to pick up another load when it collided head-on with a service truck traveling on opposite direction. The scraper operator was jarred inside the cab and reported pain to left shoulder and chest area.

Worker was moving rebar with a fork lift. The load shifted while it was in transit. When the worker unstrapped the bars from the forks, the bars came off the forks. The load was approximately waist height. The worker was struck by the load and landed on a piece of angle stored nearby.

While changing cutting edges on a WA500 loader bucket,Mechanic employee was installing bolts with hand underneath bucket. Loader was idling operator was sitting in cab. Operator dropped his hand held radio as he reached for it, his coat caught on the steering wheel causing the loader to turn and drop bucket onto mechanic employee hand.

Stripping in east quarry, a cedar tree came off the bank onto the loader across the bucket & windshield. In an effort to remove the tree from the loader without damaging the windshield, employee climbed on top of the loader bucket. As he tried to move the tree, it moved & knocked employee to the ground. His back and head struck the ground.

After loading at the 112 shovel, Haul Truck 262 passed the pole bridge and went over a rough patch of road. The truck was traveling approximately 9 MPH when operator was bounced around, jarring his back.

Party climbed out of a vehicle & as he started walking he stepped on some frozen ground that gave way with him causing him to roll his right ankle. It was a severe sprain.

Employee was descending the ladder on the right side of the loader, his gloved right hand slipped loose from the ladder handrail, causing him to fall backward to the ground. He landed on his back and had the wind knocked out of him. His back was hurting and he reported the accident to the job foreman. The temperature was 15 degrees and there was frost on the equipment.

Member placed his right foot on the addcar to unhook the cathead. When the miner operator saw the cable get tight he trammed forward causing the member to get his right foot wedged between the jack and the car. This caused three (3) hairline fractures in his right foot and was hospitalized overnight for observation.

Two workers were cleaning gob. Co-worker picked up a rock about 12""x12"" to throw into scoop bucket when a larger rock (3'x2'x5"") slid from top of the 4' pile onto injured's right hand. He received lacerations to the middle and ring finger. He refused medical treatment & finished his shift. He would not fill out a report and didn't report to work the next day.

He was leaning against the rib with his left foot against the rib and his left knee at the same height. As the continuous mining machine was trammed forward his left knee was caught by the dust box causing him to twist his left ankle. He sustained a fracture to the left ankle.

Employee was assisting the unloading of equipment from a trailer. The equipment would not start. An air hose was used to inject air into a fuel line. The air pressure blew a hole in the line forcing an unknown object into the corner of the employee's left eye.

Employee was helping roof bolt crew to bolt #7 entry, when a rib rolled and struck him. He was standing on the off side of the roof bolter between roof bolter and rib. The rock that struck him was approximately 6 ft. long x 23 inches wide x 11 - 18 inches thick.

Employee was assigned the task of breaking ice cycles in the intake roadway entry and as he was walking back to his mantrip he slipped on ice and fell to the mine floor and injured his right leg and ankle.

Employee was removing a cover from a LowTrac to make repairs. In doing so, he felt a pain in his back. He continued to work thinking it was from a previous lost time accident he had last year. He later sought medical attention including an MRI. He had another 2 injured discs and was taken off work beginning 3/28/11. He will be off work one month taking physical therapy.

Employee was working on shield #71 and using a 4 pound hammer to clean off the areas to be worked on when a small metal fragment became embedded in his right eye. Employee was wearing prescription safety glasses. The foreign object was surgically removed.

The injured employee and a maintenance supervisor were trying to lift a drill feed cylinder on the back of a mantrip. While trying to lift the cylinder, the employee stated his right shoulder popped and he suddenly felt a sharp pain. He continued to work until 4/4/11 when he had surgery to repair his shoulder.

Employee was replacing leg transducer on #92 longwall face support. When he finished repairs he reached up and pulled the spool valve handle to set the leg jack. The spool valve pulled hard so the employee jerked the spool valve handle and when he did so he felt a pain in the middle of his back and right shoulder.

Slipped on steps of coal truck while exiting the cab. Fell to ground striking right elbow and back. Transported to Summersville Regional Hospital. No witnesses to the accident.

Employee states that the hydraulic wrench slipped from a bolt and he strained the left shoulder while attempting to catch the wrench.

The injured was at 114 shield pulling in 115 shield, he heard rock fall between shields behind him. When he turned, the pontoon from 115 shield caught his right foot. It pushed on the side of his boot smashing the meta-tarsal to the top of the foot. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was a factor, miner profiency was a factor.

Employee stated he did not know how it happened but he did notice that it hurt on his way back to the Maintenance Shop from the job area. Eye began to hurt. Employee flushed eye. Employee went home, and went to sleep; when he awoke his eye was still hurting. Employee went to medical facility to have eye checked out; doctor found piece of metal in eye and it was removed.

The injured employee was roof bolting, while in the process of swinging in to put the middle bolt up a piece of rock fell and struck his left hand. This accident bruised his left hand. He was released to come back to work and continued working until 2/14/2011. On 2/15/2011 he started missing work due to swelling in his hand.

Lab Tech was crushing a sample. The buckets were heavier than usual, a 40# bucket of coal crushed into a 21# pan made for a 60# lift. Employee felt something in her abdomen 'pop'. On further investigation found a protrusion. Employee was taken to local hospital and diagnosed with urethral prolapse. This injury requires surgery which is scheduled for Wednesday, March 2 at 1:30 PM.

EE moved three 55 gallon drums (partially filled with oil). When he picked up the drums, he felt a ""pull"" in his lower abdomen. Later that evening the pain increased. A hernia was discovered and he is scheduled for surgery.

Common procedure of installing PVC slip-joint pipe was being done when excavator was being used to push a 4x4 block against the pipe to put together. The 4x4 block broke and was propelled through the air and struck employee in the leg.

IE was part of a team replacing the latch bar on a shovel. IE was kneeling on a support of the door and inadvertently placed his fingers in a pinch point while trying to get the latch bar and trip lever to align. X-rays were negative but the IE was taken off work due to the medication prescribed for pain management.

Laying track, had steel jacked up and then the steel fell off jack landing on right ring finger, received 4 stitches and a fracture to tip of right ring finger.

He was carrying a roof jack in a remote location. He stepped on uneven bottom and twisted his right ankle. The operator disputes the allegations set forth in citation # 8051131 and submits this report solely to terminate the citation.

He was wiring up shot, stepped in dust pile. There was a crack under the pile and his foot went in the crack and he fell and fractured leg.

The section mechanic had completed repairing the water supply line to the continuous miner which was located in the #4 right crosscut. As he was traveling through the crosscut to the #5 heading to return his tools to a personnel carrier he tripped on a rock measuring 2.5 x 5 x 6 inches and injured his left knee area.

Tech was lifting the turbocharger off of an engine onto the deck then to lower the turbo with his service truck crane. In doing so tech felt pain in his lower groin area and a small bulge in the lower groin area.

The employee was pushing a rock out of the way with the stab jack on the roof bolter. The rock either broke or the stab jack slipped over top of the rock, causing the drill head to move forward quickly catching his left thumb between the drill steel and the canopy post.

Worker was changing a tail pulley on a conveyor. He was using a 2-lb. hammer to align the equipment. The worker raised the hammer slightly and felt something in his lower back.

Cat 777D end dump was being loaded by an EX1900 Hitachi. Hitachi operator was blinded by the sun momentarily and the bucket of the backhoe hit the back of the end dump jarring the end dump operators neck. He was diagnosed with muscle spasms and a muscle strain of the neck and received two days of at home rest starting 3/1/11. This met the criteria for an MSHA recordable.

Injured was putting in supplemental support, during installation a small piece of draw rock fell and struck injured left arm. Injured continued working his shift, went to doctor following day and had a fracture in his radial shaft.

He took himself off for his first scheduled shift back, was re-evalauted on 2-18-11 and placed on restrictions.

Miner was installing the last row of bolts in #6 rt. While pulling a piece of rock in the face, the rib brow on the left side of the entry fell, hitting him on the rt. ankle.

EE was getting out of a front end loader when he lost his footing and fell to the ground striking his left side on the hard ground. He sustained 2 fractured ribs and contusions to his left shoulder and left side of his head.

Power line installers working at, but not for, mine. Crew was installing guy anchors for power poles when employee set an anchor attachment onto his foot. Only sprain and bruising, no significant damage or injury. Called information into 800 number as soon as we were notified.

Rebuild shop fabricator was removing shrink wrap from a pallet of parts using a utility knife. He was cutting with his right hand and pulling wrap with his left. The knife slipped and cut his left thumb. He was treated at CAMC and taken off work for a few days.

When the loader started, he climbed out and as he was climbing in, he stepped on the first rung, and as his hand reached for the side ladder rail, his hand slipped out and he went backwards and placed his hand down to cushion the fall. He was wearing work gloves at the time.

Employee was working on replacing the bags in Kiln 4 Baghouse Module 3 when his right foot got caught in one of the baghouse holes where the top of the bags are and when he went to get up from there he sprained his right knee. This did not become reportable until February 14th and light duty was given.

EE came into the reporting area after the shift and stated her back was sore after operating the shovel for 12 hrs. The first day taken off the shovels were 02/18/2011. The repetitive motion of the shovel and the personal behavior of the employee has been put on restrictions.

Employee was performing a retorque on the left rear of a Komatsu 930 with a RAD 30 gun and he smashed his finger. He was seen at the ER where xrays showed a fracture to the left 4th finger. 5 sutures required to close the laceration.

EE was walking around the back of the motor grader when his feet slipped on ice. He grabbed for the ripper and when he fell he felt pain in his left shoulder. On 3/14/11 he was informed that he will have surgery on his shoulder on 3/23/11 to repair a rotator cuff.

Tailgate on truck would not open. The driver of the truck asked grader operator to push against the tailgate with the front part of the grader. When this happened the EE was holding a rock between the grader and the tailgate his left fingers got trapped between the grader and the rock. E.R. was visted by the EE.

Employee was climbing ladder outside #5 Mill Bldg. He was about on 5th rung when the ladder starting slipping on icy surface. Employee landed on his back with ladder on top of him & his left foot caught between 5th rung & outside of ladder. He was taken by ambulance to Medical Center Hospital ER. He sustained fracture to left fibula & tibia above ankle joint requiring surgery.

Struck by falling object

Roof bolting, changing bits when a piece of rock fell hit drill pot then hit employee on top of left foot behind metatarsal. The rock measured 33""x22""x18"" thick. He was transported by ambulance to Summersville Regional Hospital where x-rays determined a broken bone in the left foot.

Contact with electrical current

Driller was raising mast and struck an overhead power line. Driller Assistant was holding wireline at the time and received electrical shock.

The employee was retrieving HV cable. As he and three other miners were pulling on the cable it snagged on a crib. As the cable suddenly stopped the employee felt a strain in his groin and stomach. After consulting a DR. on 2/25/11 it was determined he has a hernia and will need surgery.

Employee slipped on ice and fell, hitting his right elbow, and the employee felt a pop in his shoulder which caused pain. He was taken to his physician, then an orthopedic doctor and was placed on restricted duty with a sling until a follow up appointment. ONGOING.

Employee was riding in the rear of a mantrip, when the mantrip struck a bump in the road and employee was bounced into the roof and back down into his seat. Lost time injury.

EE was working to clear the pant leg from 1 Lime silo to # 2 slacker feeder belt. Lime dust was escaping from the inspection port ee had installed an air lance in to get lime to flow. The perspiration on ee's skin caused the lime to activate as the dust landed on his arm and neck. 02/28/2011 - Treated for 1st and 2nd degree burns on neck and left forearm.

A mine electrician sustained burns to his right forearm and right side of his face when he closed a 400 amp breaker and a phase to phase occurred causing an arc flash. The investigation found evidence that water had entered the breaker housing creating a common path between phases.

Employee was walking from the mill towards the office buildings, when he slipped and fell to the ground. The ground was frozen, and had a light coating of snow that had just started to occur. The employee was transported to the hospital. X-rays were taken - negative. Severe sprain to right ankle. Employee was placed on restricted duty, which constitutes this report.

Employee was drilling a hole next to a kettle bottom when it suddenly fell out hitting him on the left leg and thigh causing bruises. He was going to put a strap across the kettle bottom. He stated that he checked and sounded the top before starting to drill. He was put on light duty missing no work and returned to full duty on 3-6-2011. He was restricted for 5 days.

EE WAS AT THE CONTINUOUS MINER TALKING WITH THE OPERATOR ABOUT A NOISE THE CUTTER HEAD WAS MAKING. THE CONTINUOUS MINER WAS IN THE RIGHT BREAK OF THE #4 ENTRY AND EE AND THE OPERATOR WERE STANDING IN THE HEADING. THE CONTINUOUS MINER CUTTER HEAD WAS RUNNING AND EE WAS STRUCK IN THE LEFT EYE BY A FOREIGN OBJECT.

Employee was attempting to take off bolt from allen wrench attached to a 1/2"" IR air impact wrench. He was holding the impact wrench with right hand and his left hand was pulling on the bolt. His right hand slipped and made contact with trigger on impact wrench, causing the bolt to rotate in his left hand, causing lacerations to his left hand palm area.

Employee crushed his right 4th finger when closing rear door of spot truck. Diagnosis: open fracture, wearing splint, no use of rt hand & scheduled with Orthopedic doctor.

Employee was working on a shaker pan at the shop when he raised up and hit his head on a piece of angle iron.

Employee was cutting a piece of a conveyor belt on section 2. He was holding the loose piece with his right hand, while putting pressure to cut with his left hand, the utility knife slipped cutting thru his right glove and right thumb. He got stitches to his right thumb. Utility knife use has been changed for scissors to eliminate the hazard in this activity.

loading a bulk truck, swinging a sledge hammer to loosen prill in the bin, hurt back

Fall down stairs

While employee was descending steps, employee slipped and fell down steps.

Struck against a moving object

Employee lowered himself to the deck of a coal barge on the Spider Basket. Backlash occurred in the cable. When the employee attempted to go back down the basket dropped suddenly and injured the employee. The employee was stopped by the fall protection equipment he was wearing at the time of the incident.

Over-exertion in lifting objects

EE was installing wear plates on 988H loader when he felt a pain in his back.

INJURED LEFT THUMB WHILE SWAPPING OUT OXYGEN TANKS ON SERVICE TRUCK. THE PROTECTIVE CAGE OVER THE BOTTLES FELL TO A CLOSED POSITION WHILE HANDLING THE OXYGEN BOTTLE. THE CAGE FELL ONTO HIS LEFT THUMB CAUSING SWELLING AND BRUISING.

Over-exertion in pulling or pushing objects

EE was cleaning feeder pans and belt on feeder #3. He attempted to pull himself up and over metal tubing to reach debris and felt a sharp pain in his L shoulder.

Contact with hot objects or substances

Employee was filling can with fuel and spilt some fuel on pants. The employee then got too close to a smaller container that was lit. His left pants leg caught on fire causing burns to left leg.

Struck by flying object

Employee was working on M19 Drag Chain unbolting the bolts from a metal plate. 3 of 4 bolts were removed from the 40""x20' metal plate when the plate fell, swung around and hit him in the groin area on the right side. He was lifted about 2 ft in the air from the blow, and sustained swelling, and pain in and around that area.

Employee was lifting casing and experienced low back pain. Injury was not reported to supervisor until 2/14/2011.

Struck by flying object

Lube mechanic was striking shovel bucket dig tooth with steel hammer to drive the tooth off. A small shard of steel from tooth was sheared off & imbedded into the EE's leg. After the injury, only 1st aid & medical exam was provided. Multiple Drs advised EE against removal of the small steel splinter. At a much later date (5-3-11) EE had it removed as elective procedure.

Struck by falling object

Employee was working on the canopy of a truck box which was positioned above his head. He had been welding and pounding on it to form it back into position. He took his welding hood off to look at job and some material/dirt continued to fall and went into his eyes. The next day he saw a dr and a small sliver was removed from rt eye and drops were prescribed.

Over-exertion in pulling or pushing objects

While pulling on 60' of 4"" hose used on the super sucker truck to clean/vaccum dust off conveyor belt floor, he was standing on an incline walkway, pulling the hose with it under his arm pit.

Caught in, under or between a moving and a stationary object

Lift truck operator was driving lift in reverse and hit employee foot. Right foot of employee was hit by back of lift truck. Fracture of bones to toe on right foot.

While maneuvering a toggle plate into place the miner felt a pain in his back and when he stood up his right testicle had a sharp pain.

Employee was ascending the stairs on a hauler and slipped on the second step falling forward. He caught himself with his right hand on the hand rail and sustained a strained shoulder.
