Accident type, without injuries

At 10:30 pm a rock fall was discovered by an examiner in an area that had been abandoned. The fall measured 18' long by 18' wide by 10' high. The fall was in an intersection of the old supply road. The fall is located in #6 entry, crosscut #54, in the 1st sub panel North of 1st panel North off of 2nd Main West.

EE was walking the MMU 001 belt line and he saw the top working at spad #4+20. EE kept all personnel out of the area and pulled the section Employees outby. The roof fell in on the beltline in #4 entry. EE placed floor to roof support around the fall and moved the section.

The automatic hoist lost a footage sensor and would not operate in automatic. The hoist was operated manually until that option also stopped working. This condition was reported to MSHA at approximately 4:15 am. Upon investigation the programming for the hoist contained an error that was corrected and the hoist returned to normal operations.

A popping noise was heard in the 2WC drift-shifter proceeded to the area-pulled miners from the heading-barricaded it off-called the mine super-The Super got here-proceeded underground-was informed the area was starting to let go-The area was almost down past the ground support-he knew that it would continue to fail past the ground support-He called MSHA 800 number.

At 11:41 am on 9/27/13 the kiln system lost I/O communication causing an unplanned shutdown. The system was restarted @ 11:57 am. At 12:00 pm an ignition occurred in the primary air fan of the solid fuel handling system causing an explosion. The solid fuel handling system was shut down & inspected. No personal injuries occurred & limited damage to only the primary air fan housing.

A roof fall was discovered at 8:25 a.m. in the 6th NW Submain, # 6 entry, xc-58 spad # 42+40. 30' in lenght, 20' in width, greater than 7' in height.

A non-injury roof fall occurred on the 003 MMU during pillar recovery covering the Joy continuous miner. The rock fell from the cut and did not pull permanent roof supports.

A roof fall occurred on this date. The fall was located in the #3 entry of 3-North-1 Mains between crosscuts #10 & #11. The fall measured 0-8' in thickness and was 65' in length. The area was initially supported with 5' Hy-Tech bolts and 8' cable bolts. The fall was cleaned up and resupported.

A non-injury roof fall was discovered on 9/24/13. This is a continuation of a previous roof fall that occurred on 2/27/12. This fall is located in the #2 return entry at survey station #2708. It measures 20' to 30' long, 4' to 6' high and 18' wide. *See attached map #1 for location of the original roof fall. *See attached map #2 showing continuation of the original fall

A roof fall occurred in the 2nd Right Panel off 3SE submain at 45 crosscut in entry #10. The fall measured 20 feet in length x 20 in width x 10 feet in thickness. The area was supported with 5 foot fully grouted resin bolts.

A roof fall occurred in the 2nd Right Panel off 3SE submain at 35 crosscut in entry #10. The fall measured approximately 40 foot long x 30 foot wide x 10 feet in thickness. The area was supported with 5 foot fully grouted resin bolts.

At approx. 12:15AM, maintenance shutoff #5 Coal feeder to inspect drive belts. When the #5 Coal Mill suddenly had an internal coal dust ignition. The force blew dust up thru the feed hopper and blew off the lower inlet duct inspection door. Minimal damage with 6 hrs. to clear, inspect system and investigate cause.

A roof fall occurred on the Sub-Mains section (#13 Block, #23 Belt, #2 Entry). The fall area was approximately 16' wide and was bolted with 6' torque tension bolts and 12' cable bolts. Cribs were set at the fall location. No equipment, ventilation, or persons were affected.

An unintentional roof fall occurred in the 6 seam slope belt between footage markers 1390 and 1420. The roof fall was approximately 10' wide x 20' long x 5' tall. The operator's site specific roof fall clean-up plan in the slope was submitted and approved by MSHA. The fall reference number is 30F.

Rock fall was found at break 20 adjacent to #2 belt intake in the #5 entry. Fall measured approximately 20' wide x 20' long x 8'-10' thick.

A fall was discovered on E-1 Panel in the #1 Entry that was an alternate escapeway. The fall area measured 184' long and was bolted with 6' and 8' SuperTwist bolts and 16' CC cable bolts. No people, equipment, or ventilation was affected. The escapeway was re-routed.

At 3:35 pm IE reported to (PCS)that he was stuck in the passenger elevator of the mill building PCS called out an electrician at 3:40 MSHA was notified at 3:45 and elevator company notified at 3:50. Electrician arrived at the plant an 4:20 and employee was out of the elevator at 4:24pm Thyssen Krupp reported to PM elevator on 9/23/13

During the weekly examination at this idled operation, five areas experienced a fall since the previous examination. Primary support bolts (8' torque tension bolts with resin) were pulled in the areas during the fall. The areas had been previously cribbed. *A map showing the exact location of the falls will be provided via fax to the Denver office.*

Located at Zone #1 6740 crosscut 5, a 15'X15' section of the back fell to the floor. A pile of material approximately 10' high was deposited onto the floor. No miners present at the time of incident.

At approx. 3:20 pm, a roof fall above the anchorage was found in the intersection of the A entry of the 4North 6Left section. The fall was dangered off on all sides and the return was re-routed. There were no injuries or property damage. The fall was approx. 70'long x 18'wide and 6.5'high.

At approx. 6:30 pm a roof fall above the anchorage was found in the 6left 4North track entry at spad #12+71. The fall was approx. 40'long x 18'wide and 8'high. The fall was dangered off on all sides and extra cribs were set. There were no injuries or property damage.

During a rain event the incoming power to the mine experienced numerous surges/spikes that caused the power to drop several times. Those surges eventually caused 2 of the 6 fuses protecting the slope hoist circuit to fail. When it became evident that the repairs would take longer than 30 minutes the event was reported to MSHA at 2:36 pm.

At 6pm it was reported to safety that the right rib was sloughing at the mud seam. Barricaded the area off. At 9pm after an inspection of the area the peeling had continued until the roof support let go. At that time called MSHA 800 number to report the incident.

Maintenance was patching holes by spot welding to the bottom of an enclosed belt conveyor. The heat from welding transferred through the 3/16"" thick steel substrate to the UHMW lining and to the rubber cleated belt with sidewalls causing it to ignite and catch fire.

An unintentional roof fall has occurred on the #2 Section Left Return break 15 spad 6677. The roof fall was approximately 40ft x 18ft x 4ft. There were no injuries reported and no equipment involved.

The hoist-man was preparing to start skipping muck by balancing the skips according to the level they were pulling from. During this process the hoist kicked out due to failure of the main field breaker. It tripped and would not reset the hoist. There were no personnel on either conveyance at the time. The breaker was replaced and the hoist was re-commissioned.

Operator heard a loud pop from the engine compartment and the Cat D11T Dozer became engulfed in flames.

A fall was discovered in the tailgate entry of the 7B Longwall. The fall was at survey spad 16+10. The fall was above the anchorage and did affect ventilation.

While mining an auger hole (uncharted) was exposed with the continuous miner a large amount of water came out of the exposed hole. Apx 18,000 gallons.

A roof fall was found by EE on his weekly air way examination. The fall was found 1 break inby spad #1129 (3000 feet outby). The fall is approx. 55 feet long - 18 feet wide and 8 feet high. This mine is idle and has been since Nov-12.

The elevator at #1 portal was out of service for maintenance. As the elevator was being ran control cables became entangled with elevator counter weight. The elevator would not operate at this point.

A fall was discovered in the Sub-Mains Section (#4 Head, #13 Block) on the #23 Beltline. The area of the fall measured approximately 20' x 17' x 17' and was bolted with 6' torque tension bolts and 12' cable bolts. No people, ventilation, or equipment was affected.

Miner loader shuttle car started to load 2nd car in #6 heading when had ignition in face. No workers were injured. No Equipment damaged was only a flash with no duration. Investigated by MSHA.

An employee was working on removing a blockage in a muck chute by blasting. After several days with no progress he decided to create an opening to allow muck to bypass the blockage. This allowed material to accumulate at the bottom of the raise. He did not remove this material before deciding to wash additional material out of the chute which eventually obstructed the access.

IGNITION - Occurred when the continuous miner was cutting the bottom rock on No. 11 section in No. 3 entry when a methane bleeder ignited on mine floor. Ignition lasted approx. 20 seconds and used 1 fire extinguisher and water hose.

Dozer fell through a void in the stockpile trapping the operator for approximately 2 hour before rescuer could uncover and free the operator from the dozer.

A roof fall occurred at the #21 crosscut, in the #2 entry spad number 2112. The fall was approximately 16 feet high by 80 feet long by 25 feet wide.

There was a unintentional roof fall located on the 8th 48 beltline, between x-cuts 29 and 30, in the straight. It was approximately 50 ft long, 18 ft. wide and 8 to 9 ft high. 84 inch tension rebar bolts was used in this area.

Crews were transported underground without event. The Hoistman began the required brake tests & identified the Pinion Brake. A Brake test rating of 1500 could not be attained by a small margin. Crews were idled while the corrections were made. Maint came up the secondary & buffed the contacts. Normal hoisting was resumed and crews were released for work.

An unplanned roof fall occurred in Wyoming #2 mine in th3 #6 entry, at #49 crosscut, at spad #600 that measured 12' long x 5' high x 20' wide. No injuries were reported. The area was developed on 1/1/08.

A battery powered personnel carrier was parked U/G and later discovered on fire, requiring employees working in/by to don SCSR's and evacuate out/by to a safe location.

While conducting his weekly examination a fireboss identified a roof fall in the Left return #1 entry of Detour Mains at brk #84 near spad #163.The fall measured approx. 120'L X 20'W X 4'H the area was supported with 4' fully grouted resin bolts and mined approx.7-2010. The fall will not be cleaned up and the area has been dangered off and timbers set.

An unintentional roof fall occurred on the 5 West Long Wall at crosscut number 60, entry number 2. The fall measured 22 feet in length, 16-18 feet in width and 20-25 feet in height. The area was supported with 12 and 16 foot cable bolts, T-3 channel, and wire mesh.

A roof fall was found in the #5 entry at crosscut #40 which is the mines primary escapeway. The fall was estimated to be 18 feet by 40 feet by 10 feet in diameter.

An unintentional roof fall was found by mine examiner one break inby spad 1039 in the #7 Entry of #1 SE Mains at approximately 11:45am. Fall measured 18x24x6 1/2. All approaches have been cribbed/timbered and dangered off. Ventilation not interrupted. MMU's in non-producing status since August 16,2013.

EE went into the intake to look at the tracking and communication equipment and found a roof fall. EE called out to another ee called in the roof fall to the hot line. The roof fall was in the intake in #7 entry at the 1st North West submain at spad # 1+70. The fall was 15 feet wide 26 feet long and 7 feet high. The fall was called in at 4:15 p.m.

ENLOW #1 ELEVATOR WENT DOWN WITH NO ONE ON BOARD. MECHANIC HAD TO CLEAR FAULT ON MAIN DRIVE TO GET IT TO RUN.

A fuse was blown on the hoist and a bad switch was found. Miners were evacuated from underground. A new switch was installed and the hoist was tested. Miners returned to the underground area to finish the work day.

Roof Fall was discovered on 1 North Mains Break 56 to 57 on Track Entry. Fall measured approximately 18' to 19' wide x 60' to 65' long x 3' to 5' high.

A roof fall was found in our M3 Mains. The fall is specifically located at x-cut 56 in entry #2, a primary escapeway. The fall measured 18' x 17' x 15' high. The fall did not impede passage of miners, affect ventilation, or cause injury to any miners.

The 24 volt DC to 120 volt AC inverter went out. The electricians had to install a new one.

There was an electrical storm in the area. The Plant had a power surge and the DC high speed breaker kicked out on the Service Hoist. The time that the sequence was completed for re-setting the breaker was one hour and twenty minutes. The Hoist became operational at 8:20 a.m.

A mulching machine was clearing a pipeline right of way. The mulching machine contacted a pine tree and the top of the tree broke off and fell onto a set of power lines. An arc from the power line started a fire which spread rapidly. Three fire departments and the forestry service responded. The fire burned approximately 4 acres and was contained within three hours.

The #1 Elevator was stopped at the top of the landing and could not be called to the bottom. The computer room operator was contacted at 12:25 AM. The computer room operator then contacted EE around 1:00 AM and he found that the dynamic brake had a PLC Fault. EE then reset the fault, tested the car and returned the car to service at 1:24 AM.

Service hoist went down at 4:15 am. PLC processor card failed. Processor needed to be replaced and software reinstalled.

A fall occurred in the Sub-Mains Section (#11 Block, #7-#9 Heading) on the #23 Beltline. The are of the fall was 130' x 16' x 20' and was bolted with 6' torque tension bolts and 12' cable bolts. No people, ventilation, or equipment was disrupted by the fall.

It was about 3:00 PM when we found a roof fall at x-cut 66 in the number 3 entry of the Headgate #3 unit.

A fall occurred on the #7 North Section (#2 Heading, #12 Block) on the #17 Beltline. The area of the fall measured 16' x 16' x 20' and was bolted with 6' torque tension bolts and 12' cable bolts. No people, ventilation, or equipment was disrupted by the fall.

Rock Fall was discovered on the 1 North Mains BK 41 #3 Entry. Fall measured approximately 40' long x 20' wide x 6' thick.

Hoist became inoperative at approximately 1:45am. Surge Suppression warning was indicated. Hoist engineer reset fault and made several test runs with hoist prior to operation. Issue was resolved at approximately 2:50am.

Unplanned roof fall occurred on the 3rd N.W. Submain belt line entry #5. Spad 55+80 It was approx 60' in length, 18' in width, and greater than 6' in height.

An unintentional roof fall has occurred on the #3 Section #2 Entry spad 14837. The fall was approximately 90' x 20' x 8'-12'. No injuries were reported. The roof fall fell above anchorage point which covered up the continuous miner. A recovery/clean up plan has been submitted.

Roof fall was discovered in South East Mains during weekly examination. The fall was approximately 18x40 in the # 1 entry, 80ft outby spad# 506.

A miner was using an excavator to remove concrete footers and severed one or more underground utility lines. This resulted in the miner being confined to the cab of the excavator until the area was cleared and safe for the miner to exit the excavator. No injuries to the miner and no damage to the excavator.

An unplanned roof fall was discovered at 23 break along 2 Right #1 belt in the #7 entry. The fall measured 25 feet long x 25 feet wide x 5 feet thick. The fall did not disrupt ventilation or travel.

A power outage occurred at 5 Shaft at approx 3pm on 9/2/13. Recloser #1 at MOS #3 sub tripped and locked out; this is suspected to have occurred due to storm activity in the area. #5 Hoist lost power just as it began hoisting 14 employees. Power was restored about 2.5 hours later. After hoist checks were completed all employees were then safely raised to surface.

A roof fall occurred adjacent to the #6 belt in the primary intake airway at crosscut #49. The fall measured 25' long, 20' wide and 6' high. No equipment was involved and no injury occurred.

temperature were around 70 degrees, sunny and dry day. Adding to narrative per prelim **The victim was operating a 2 1/2 ton truck up a steep roadway. He was hauling water tanks on the bed of the truck when the truck overturned on its roof and crushed him.**

Struck against a moving object

On Monday 9/16/13 at 09:25a.m. a pit/haul truck driver, in truck number 22-2012, with 25 years, 42 weeks experience turned left and crashed through the protection berm and fell 80 feet to the pit floor

Caught in, under or between running or meshing objects

Employee was operating the hydraulic core press, right thumb was caught between the ram and the core tube.

Employee was checking a leak near an oil plug on the C boiler ID fan in the Powerhouse when he reached up under the coupling gaurd and came in contact with the RPM sensor resulting in fractures and lacerations to his left hand and fingers.

As Employee prepared to operate locomotive he put his finger between frame of motor and shield. As he sat down he placed his bodyweight on shield smashing his right middle finger. The accident was not reported to Marfork Coal by the injured. An internal investigation was conducted and accident did occur on the job. On 11-22-13 finger was amputated due to severe infection.

While running roof bolter he cut his ring finger tip off when lowering the boom had hand on the boom when pot came down cut the ring finger tip off and smashed middle finger of the right hand

Caught in, under or between running or meshing objects

EE was drill steel on a drill. He was using his boom to hold the steel with a nylon strap. He was holding the strap and his hand was between the steel and strap. The steel caught on the striker bar and started turning the strap which wrapped around the steel while it turned it grabbed the tip of left little finger and jerked it off.

Over-exertion (Not Elsewhere Classified)

Employee was coming out of mandoor and stepped in a hole and twisted left knee. Employee did not start missing time from this injury until 10/04/2013.

Over-exertion in lifting objects

Employee was picking up plaster buckets when he twisted he felt pain in the middle of his back.

Struck by flying object

The IE was changing bits on the continuous miner, and as he was attempting to remove the bit with a hammer a small piece of metal broke off the shoulder of the bit, and struck the employee in the right leg and embedded into the quadricep muscle.

Struck against a moving object

The employee was operating a battery coal hauler to haul rock dust when his seat bottomed out. The employee complained of pain in his lower back and left leg but continued to work through October 14, 2013. The employee began missing work on October 15, 2013.

Struck against stationary object

The employee was responding to a call-in, while walking through the roller mill building employee stepped on a medium sized stone, slipped and extended his left leg. Employee felt a twinge in his left knee and upper left thigh, but no pain until the next morning. Note: Employee did not seek medical attention until 11/6/13 and first lost day is due to surgery on 12/10/13.

Rubbed or abraded

Employee was on top of a joy 14-09 continuous mining machine plumbing hoses into a bolter module. He was working from his knees when he felt sudden pain in his left leg and knee. When he tried to stand up he was unable to straighten his leg. He was removed from the mine and sent to the hospital for evaluation.

Over-exertion (Not Elsewhere Classified)

Employee was cleaning the prescreen when dirt fell on his foot when he pulled his foot out of the dirt he twisted his knee. We took him to the ER the next day for X-Rays the did not see anything wrong. He followed up with a different DR who wants to do a MRI and put the employee on modified duty.

Struck by falling object

A rock fell from the mine roof near the face of the #5 entry approximately 100 feet inby spad #13280 on the #3 Unit, striking the employee on the head and neck.

Fall from machine

Employee was climbing into the D-11 dozer when he slipped. When he reached for the grab handle, he felt pain in his stomach area.

Over-exertion (Not Elsewhere Classified)

Employee was plugging a scoop battery up when he felt pain in his back. He continued working until October 7, 2013 at which time he began losing time.

Struck against stationary object

Employee was working on the 1C Belt drive, and as he was walking around the tailpiece of the 1B belt, he met a group of miners. As he was walking to avoid traffic in the congested area, he stepped on a anchor pin that was sticking up out of the ground approximately 3.5"". This action caused him bruise the bottom of his foot.

Fall to the walkway or working surface

St. Paul Builder/Block vendor was unloading a pallet of block when he stepped backwards and stumbled over the out rigger and landed on his hip resulting in a fractured hip.

Struck against stationary object

EE was working on a de-watering pump in the return when he slipped on a wet rock and twisted his knee. EE worked his scheduled shifts and went off work on 1/7/2014 for surgery to repair a torn meniscus.

Over-exertion in pulling or pushing objects

EE was roof bolting, when he pushed the roof bolt into the hole he felt pain in his right hip.

Over-exertion in lifting objects

Employee bent down to pick up a jack for a motor and while picking it up he dropped to one knee and said his back was out.

Caught in, under or between a moving and a stationary object

The injured got his right index finger mashed between a roof bolt plate and the top as he was installing a roof bolt

Struck by falling object

Plastering a stopping bent over to open another bucket of plaster a piece of draw rock fell striking him in the middle part of the back.

Struck by... (Not Elsewhere Classified)

Crew was advancing shield. Top rolled off shield onto mesh and the boat winch spun free. The handle hit employee in the face.

Flash burns (welding)

Employee was washing a track on a dozer, dirt flew back and hit him in the eyes, also that day he had been helping the welder on the job site. Employee finished shift and went home and his eyes started burning went to the ER where he was seen and had his eyes washed out. The ER doctor stated this could have been a flash burn from the welder.

Fall to the walkway or working surface

Injured employee was attempting to dismount a rail mounted personnel carrier to throw a switch when he tripped and fell to the mine floor, straining his right shoulder. Injured did not begin to miss work until 12/16/2013.

Over-exertion in wielding or throwing objects

Employee was using channel locks to pull belt up off of mine floor when he noticed pain in shoulder, he started training belt and when he swung a hammer he felt a pop in his shoulder causing a torn bicep.

Struck by falling object

Miner was greasing head pulley on screen plant. He fell off the plant frame approximately 6' to the ground surface, landing on his back, he laid on the ground conscious but in pain. He was transported by ambulance to Jordan Hospital ER. He was released later that day. He had severe bruising that kept him out of work the remainder of the week. He returned to work on 9/30/13

Employee was initially released to return to work by Ruby Memorial Hospital. Further investigation determined that the employee is being kept off work by a physician in southern West Virginia.

The employee returned to work the following day. The employee continued to complain of pain in his shoulder and went back to the Dr, and has been put off work.

Installing a new conveyor belt on a concrete sand stacker. While unrolling the belt that was on a metal shaft, the employee placed the belt on the forks of a forklift. The roll twisted off the forks striking him in the right leg causing contusion and bruising to his right leg inside the knee area.

EE was watering the quarry roads. As he was driving down on of the roads he tried to use the brakes to slow down he says the brakes ""failed"" and he was unable to stop. He managed to turn the wheels towards the embankment was of the quarry and was able to stop the truck that way.

Co-EE and IEE were carrying a nine foot track rail, when they threw the rail down, the rail bounced up and hit IEE on his right foot.

Employee was attempting to make ends on a belt splice. Before he could begin, he noticed that one end was trapped beneath the rest of the belt. The employee tried to free the end by pulling with channel locks when he began to experience pain in his back.

After putting in test bolt, he leaned over the drill head to examine it for obstructions (065 MMU, right side bolter station of Joy Miner). He put his right hand by the bolter mast and lowered the timber jack with his left hand. The timber jack pushed the drill mast down pinching his right hand, breaking the distal phalanges of both the thumb and index finger.

While attempting to remove a tilt cylinder from a push arm from a D11 the tilt cylinder became lodged. This caused the push arm to swing out and hit the contractor on the foot. This resulted in a bruised foot.

Bates Contractor was riding Mac 8 man trip to the 001/002 section. The ride hit a bump causing him to have severe pain in his lower back and left leg.

The injured person said he took a bar and was prizing area real hard on the ARTS shields to expand them and strained lower back. Then when he started to crawl his back hurt real bad. He came outside.

He was exiting the cab of truck to do his walk aground when he lost he's footing and could hold on and fell to the ground.

The employee was pulling on the miner cable to get enough slack to hang the cable when he felt pain in his left forearm and elbow area. The employee continued to work until having surgery on February 6, 2014 to repair a torn tendon in his forearm.

While installing truss bolts the injured was pushing a drill steel in a hole when he felt discomfort in his right shoulder. After a doctor's exam and a MRI it was determined that the injured has degenerative conditions that require surgery. This incident is being reported as an occupational illness's. The injured worked toll 9/11/13 when he went off for surgery.

Employee was cutting a burn ring when a piece of hot slag entered into his boot, causing small burns to his left foot. Employee treated with his personal doctor on 9/26. He was taken off of work and missed his scheduled shifts on 9/25-9/27. Employer was made aware on 9/30 that the missed time was related to his to his reported 9/5 work injury.

Injured employee was installing rib bolts on the 7 Right section. As he repositioned the bolter after installing a bolt a piece of rock measuring 10-12"" long by 3"" thick fell from the roof and struck his upper back resulting in a strained and bruised back.

The plate was not just bent upwards into the section as usual; it was broken completely and only being held by debris. As the plate fell towards him it struck him on his shoulder and neck area.

He was loading sand props into a sled. He was pulling on a sand prop to re-position it in the sled when he felt pain in his right elbow. An MRI revealed a tear in his right biceps muscle. He began to miss time on 9-6-13 due to his injury.

Flash burns (electric)

Testing a control panel on a dewatering well pump. Arc flash occurred, burning the employee on the hands and minor burn to the face. Ee transported by ground ambulance to the ER, diagnosed with 2nd degree burns on the hands and referred to the outpatient burn unit.

Employee was pulling cloth over on 2B in the seed wash unit when he pulled something in his lower right groin area. He was diagnosed with a hernia and is unable to return to work at this time.

Employee was standing under a shield tip to hang mesh onto the shield canopy. When a coworker lowered the adjacent shield to allow the mesh to be hung, a rock slid off the lowered canopy and struck the employee on the right hip/back causing the employee to stumble backwards and hit the longwall face as he fell to the ground in the area between the pan line and the face.

As the roll broke over the edge, the employee stopped it briefly and then let it go but attempted to slow the momentum of the fall and therefore the weight of the roll jerked his wrist from the weight causing pain.

Employee was dismounting a Caterpillar 769C haul truck when his left foot became wedged between the bottom of the bumper and the factory mounted hanging bumper step. Right foot slipped off the bottom step. This action caused the employee to lose his balance and 3 points of contact and he then fell to the ground. The doctor immobilized his leg and an MRI will be done on 10-4-13.

He was bending over to place his lock on the lock box that was located on the right shoe and his foot slipped due to mud on the shoe. He fell forward off the shoe about 7 feet to the ground fracturing his right wrist when hitting the ground.

Employee was filling the hydraulic system of the 992 C Cat Loader. Employee finished filling the loader and was coming down the ladder. When employee got to the 3rd step from the top his left foot slipped off and he began to fall. While falling, employee's right elbow struck the ladder rungs causing a contusion to his right elbow. Employee was off work and restricted for 4 days.

The employee was operating a battery coal hauler when the canopy of her vehicle made contact with the roof. A piece of rock from the roof entered the operator cab and struck the employee which resulted in contusions to her back.

Employees were transported to Campbell County Memorial Hospital and later flown to the Western States Burn Center in Greely Colorado for the best immediate care with non life threatening injuries.

On 9/10/13 a fuel/lube tech was standing on the right side bumper of a FE loader assisting the operator to bleed the fuel system. The operator's hand slipped while spinning the fuel filter. The fuel/lube tech was bumped, causing him to fall off the bumper (approximately 4') to the ground. On 10/22, the fuel/lube tech had a medical procedure that makes this incident reportable.

He didn't close the fan louver's and put a board in the fan to keep it from turning as a safety, the fan started turning backwards he tried to stop it with his right foot. Employee was driving a haul truck and after loaded with oversized rock, the front of the haul truck lifted off the ground and back to the ground. This caused the employee to have lower back pain.

Employee was in the process of removing a bracket that supports wire inside a switchgear cabinet. While pulling on the old bracket it unexpectedly broke causing the injured employee to strike the back of his hand on a small bolt that was protruding through an adjacent hinge.

Ee made claim of low back pain at start of shift after picking up chock block. Seen at the ER, diagnosed with a strain and released to modified duty. After investigation of the claim by workers' comp carrier and mine site, it was determined to be work related on 10/07/13.

The employee was tramming a battery powered scoop in an outby entry. While driving the scoop back to the unit it bounced and the injured hit his head on the canopy straining his neck. He was taken to the Dr and put on restricted duty.

Employee was replacing a pulley on the primary crusher. He felt something ""pull"" in his upper right arm, but didn't think much about it. He had some swelling the 2nd day and it was still bothering him, so he notified the plant superintendent. He went to the med center and was diagnosed with 727.62 biceps tendon rupture. He is scheduled for an MRI at a later date.

Employee was traversing from one scaffold platform to another when he miss-stepped. His foot was caught between platforms when he twisted his body and injured his foot.
