Accident type, without injuries

An unintentional roof fall occurred on the S2J in the #9 entry of the 21st Xc. The fall was approximately 25' x 18' by 8'. A plan was implemented to add supplemental support to the area.

A roof fall occurred in the #1entry of North Mains at 48 x-cut. The fall was approx.35' in length,18'wide and 9'high.Area was bolted 8'point anchor resin assisted bolts, supplemental support consisted of bacon straps, wire mesh. Cause of the fall was deterioration of roof due to weathering.

A non-injury accident occurred when the slope car malfunctioned. Production was halted and people withdrawn from the mine. Both escapeways were available at all times. NO persons were injured as a result of this mechanical malfunction. The malfunction was corrected at approximately 3:00pm.

A Non-injury Roof Fall was found During a Weekly Exam on 10/30/2019 on the 1st East Mains Return at Break #17. The Fall is approximately 20 feet Long and 20 feet Wide, High has not been determined. There was no injury's associated with this fall.

At approximately 4:50am there was an ignition in the #2 entry of the 20E section. There was no mining going on and the crew were bolting the faces for the weekend. As the roof bolter operator lowered drill pot from the bolt. That is when the ignition was seen and it only lasted approximately 15 seconds. There were no injuries during this ignition.

A grass fire was reported to the guard shack at 3:36 pm. Mine employees began fighting the fire and requested help from Craig (town). No buildings, equipment or personnel were in the path of the fire. The fire was extinguished without injury.

A small sulfur fire ignited spontaneously in an inactive pit. No persons or equipment had been working in the pit for some months. It was determined to restrict access to the pit, provide gas detection monitors to personnel in the vicinity, and to allow the fire to burn itself out.

The automatic door sensor for the Pleasant Grove Elevator was not working and needed replaced. No one was on elevator during time of repair.

Smoke was noticed at 11:45 at N-8 hopper and found a small spontaneous combustion fire due to location the fire burnt for more than 30 minutes before it was extinguished.

An unplanned roof fall was discovered adjacent to spad # 5303. The fall is limited to one intersection and measures approximately 18.5 W x 19 L x 5-7 T. The fall fell above anchorage.

Employee struck a power line with the bed of haul truck. EE exited the truck 43 minutes later after it was confirmed the power was deenergized. A citation was issued (9471229) for failure to immediately report on 2/7/2020. The citation was contested and upheld.

An Unintentional roof fall occurred in the #5 Entry outby brk 112, in the intersection @ Survey Station 23620. The fall was approximately 12'=H X 18'=W X 15'=L.

An unintentional roof fall occurred in the #7 entry of 6 south mains area. The fall occurred at the 59 x-cut. The fall occurred as a result of adverse geological conditions that deteriorated over time. The fall dimensions were 8ft wide by 6ft high by 50ft long. The fall occurred out-by the active section with No equipment damage or injury as a result of this event.

A roof fall was found measuring 4 1/2'high X 14'wide x 30'long. The fall has been dangered off at all approaches. The fall does not impede travel or ventilation.

An unplanned and reportable roof fall was discovered in the D-Mains #3 entry at the 28 block 2 to 3 intersection on 10/21/2019. There were no injuries, no equipment damage and the ventilation system was not disrupted. This occurrence was immediately reported.

On 10/21/2019 at approximately 3:17 p.m. a roof fall was discovered in the #2 entry at 17 break along 3C belt in the left return off of #3 section.

The ground failed to a height of approximately 5 meters in a 26 meter stope. The area was previously barricaded off with no active mining taking place. Future plans were only to blast and remote muck with no personnel ever entering that area. The stope was supported by using 8' bolts and 20' grouted cable bolts at the time of the failure.

At approximately 9:30 am the service hoist on the Camp Creek side went down due to a E-Stop relay fault. During the investigation on the cause they found a bad wire going to the 24v card located in the PLC cabinet. The repair was made and the hoist was put back in service at 11:40am.

An unplanned roof fall has occurred at crosscut 37 on the cedar grove seam in the track entry. The fall measures approximately 40' long and 10' high.

Spontaneous combustion smoker was found in the MW pit. Had to push dirt over the top of it. The area was buried at 10:00 am 10/17/2019.

ROOF FALL - Occurred on E-12 No. 2 Longwall in No. 1 belt entry from cross cut 33 to 33-1/2. The rock fall was above roofbolt anchorage and was approx. 60 feet long, 20 feet wide and 10 to 14 feet in depth. No one was injured.

While drilling in Unit 12, a small pocket of methane was encountered. This lead to a reading above 2 ppm at the end of a drain hose from the drill. The methane cleared from the area within seconds, readings taken in the surrounding area were non-detectable.

An unintentional roof fall occurred in the 3 North area #5 Entry 187 wall. The fall measured 16' Width X 24' Length X 13' High. There were no injuries as a result of this incident. Conditions contributing to the roof fall is adverse geological conditions in this area.

Roof fall at 5 XC Main South, collapsed overcast. Minimal ventilation disturbance. When issue was found, the mine was evacuated through 2 shaft. No gases were detected, no injuries.

The Kuhntown elevator was transporting three employees to the surface when the safety switch on the surface door failed, causing the employees to become entrapped approximately 240' from the surface. Once the safety switch was replaced the employees exited the elevator at 1:34am. It did not involve any injuries, equipment or compliance issues.

An unplanned roof fall occurred in the right return at break 9 on #6 belt in the vicinity of Spad No. 1011. The fall measured approx. 20' long x 16' wide x 4 1/2' thick.

Examiner was checking water pumps and found a roof fall in the #5 entry intake, Break 69. The area had already been cribbed and dangered off. The fall was contained within the cribbed area.

An unplanned and reportable roof fall was discovered on 10/09/19 in the 31 Butt #2 entry at the 4 break 2-3 crosscut on the outby corner. There were no injuries, no equipment damage, and the ventilation system was not disrupted. This occurrence was immediately reported.

An unplanned roof fall was discovered in the Main North #3 entry at the 86 block overcast row. The fall was approximately 12' wide, 40' long and 12' high. It was most likely caused due to gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance.

Unplanned roof fall at Break 33 in the #4 entry. The fall was dangered off and cribbed at all approaches. The life line was rerouted around the the fall in a different entry.

A roof fall was found at 08:00 AM in a 3 way intersection of entry #5 MS10 XC14 station #22+84.95 intake travelway. No injury or property damage occurred, travelway is being re-routed and access was propped off. Fall was approximately 30'x20'x10' area was mined in Oct. 2002.

Employee in elevator car, when elevator car reached desired floor the elevator door would not open. Employee contacted operator via radio, communications maintained at all time, elevator car was locked out, fall protection worn and anchored, used the elevator door key and opened door, employee walked out of elevator. Time in elevator car approximately 35 minutes.

A three way intersection in the primary escapeway of the 4 Right Longwall section fell above the anchor zone.

Fueler noticed flames coming out of the engine compartment while driving on the tailings facility causeway. Employee reacted as trained and proceeded to call a MAYDAY, put the truck in park, set the park brake, shut the engine off and initiated the fire suppression system, and exited the truck. In the process of walking away from the truck flames reappeared.

An unintentional roof fall occurred in the #5 entry break #187 on 3 East Mains in the intersection at Survey station 29974. The height of the fall was approximately 10 ft high and fell towards the #6 entry for about 10 feet.

On Oct 2, 2019 we had a washout of a dike holding back our silt due to rain on Oct 1, 2019 and on Oct 2, 2019 with all the rain we had the 2 days on. Prior we had to shut down the wash plant at 15:00 on Oct 2, 2019. Sometime that evening/night the water level rose too high and started to run over bank, the rain continued and eventually washed out dike.

A person was driving on state route 245 in Bardstown, apparently had a health issue and ran into our fencing.

At approximately 2:10 pm, a fire was discovered on a Polaris EMU that was located at the coal fork elevator bottom area that was not extinguished within 5 minutes.

A non injury incident occurred when the axle to the weight car broke. All personnel were withdrawn from the mine. Both the primary and alternate escapeways were available for use. NO PERSONS WERE INJURED. The weight car was removed and the slope car was returned to service approximately 50 minutes later.

While employee was working on the ride a piece of rock rolled off coal rib, striking them in the back. Pinning them against the personal ride.

EE operating west starter machine when hanger bar came out. Pushed emergency stop before machine jammed. EE asked coworker help reverse machine. EE put right hand on sheet feed platform to watch. As machine rotation reversed sheet guide pulled EE's right middle finger amputating the tip of middle finger distal phalange from 3/4-to full fingernail up.

On Thursday, November 14th, the employee reported to the site supervision that they were experiencing pain in abdomin due to overexertion from a task performed October 28, 2019. Employee alleges they felt a small (nickel sized) ball behind belly button they felt on Tuesday, November 12th at home.

3 miners were running cable in the 926 Fun Zone Down Ramp; requiring a new cable spool on a stand they attached Vic clamps on each side of the cable spool to keep the spool centered. 2 miners pushed the spool to allow room to install the VIC clamp which caught the tip of 1 miner's ring finger which was caught between the spool and the Vic clamp removing the tip of the finger.

Two employees went to drop the lid on a tool box, employee tried to catch a tube of grease that was falling from the box when a finger was caught between the lid and the side of the tool box. After the initial treatment for a laceration, the ER Physician referred them to be evaluated by an orthopedic surgeon. The first joint of the right index finger was removed by the surgeon.

Mounting tire on L330D loader. Using crane boom end to depress tire to install lock rings when our service vehicle shifted on its out riggers causing the boom end to move upward pinching employee's hand between the boom and loader wheel. This contact resulted in the partial loss of employee's left middle finger at the first joint.

Employee was poking a hole in the K1 tower using a bar and a 12.5 lb. sledgehammer. The employee who was injured was holding the bar, while EE's partner swung the hammer. The hammer missed the bar and struck the employee in the left ring finger. The injury resulted in the removal of the tip of the left ring finger.

A Technician was in the process of replacing a rear strut on a 798 haul truck. As they removed the lower rear strut pin, the lower section of strut fell. This was caused by an unknown internal failure which allowed the lower section of strut to fall hitting the Technician in the leg.

An employee was troubleshooting a flow issue on a volumetric feeder. During the course of troubleshooting, an access hatch was opened and the feeder was jogged. At some point, the employee's hand entered a pinch point (rotary veins) before the feeder had come to a stop. The employee sustained partial amputation injury to 3 fingers.

I.E. was working on moving up sights for the next cut. As EE moved the line curtain away for EE's laser to advance up toward the face, a rock fell out between the bolts and drove EE to the ground. Rock measured 85""Lx53""wx12-14""th. Evident injuries to right arm and right leg.

A fully trained miner was assisting a center bolter when a piece of rock fell between the straps and struck them in the right arm. The injury was not the result of any compliance issues nor were any protective guards involved.

Mechanic was using come along to hold tension on a steel bar as they heated it with a torch , the weld broke on bar and it came back and hit them on the left leg.

WORKER WAS CLEANING OUT DEBRIS CATCH BETWEEN THE FRAME PLATE AND THE CHOPPER RIG LOWER PLATE ITSELF. WHEN DEBRIS WAS REMOVED THE CHOPPER CYCLED AND THE FLEXING ACTION OF THE TWO PLATES CAUGHT THE WORKERS LEFT HAND FINGERS BETWEEM THEM BEFORE IT COULD BE WITHDRAWN. THE MIDDLE FINGER AND THE RING FINGER WERE BOTH MASHED NEAR THE NAIL.

Employee was walking off the longwall face when EE felt something give way in the right calf. EE was diagnosed with a torn right calf muscle.

Employee was using a slate bar to remove a rock from the Right CM on 006 MMU. While pushing down on the slate bar to slide the rock off the slate bar slipped out of employee's hands causing the bar to hit employee in right eye.

IP entered a tank that was off limits to employees. IP entered tank while heated water was flowing into tank to remove built-up sand. IP lost footing and fell into water causing burns to legs and abdomen.

The employee was setting up the main breaker on BT800 locomotive when EE felt pain between the left index finger and thumb. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. Employee began missing work on 11/04/2019.

While EE was holding cutting edge to equipment bucket by hand another EE was trying to remove torch cut bolts with a pneumatic hammer. The hammer slipped off the bolt and hit the EE's left ring finger causing a displaced fracture to the medial phalanx.

Employee was climbing into shaft bucket for a man trip. Employee placed left foot onto the bucket bracket and stepped over the rim of the bucket and left foot slipped off causing employee to straddle the bucket rim landing on groin.

Fall to the walkway or working surface

EE was walking to the door to enter the warehouse. They looked over at two other miners while walking, left foot caught the corner of the outside floor mat and they tripped. EE was carrying paperwork and when they fell to the floor they landed on their left elbow.

Struck by powered moving object

EE was function testing shields when the pan line was pulled back coming into contact with a knee. EE did not seek medical attention till 11/7/2019 becoming a RLT on 11/11/2019. Cause of this incident was lack of EE concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency may have been a factor.

While an employee was being loaded underneath the shovel, the employees neck and back were jarred when the material was loaded into the back of the haul truck.

The employee was hooking up a rope to a piece of rock to pull it off the miner at that time the employee got their finger caught between the rope and the ram car clevis.

Employee was climbing down form a rock truck. They slipped and fell.

Employee reported on 10/31/2019 that they had allegedly strained their right shoulder while using a hose at conveyor on 10/30/2019. Went to ER and was released to return to work: 11/1/2019 went to ortho and was given 2 weeks off.

Facility had rain after several days and floors were wet causing the employee to slip and bend knee backwards.

Struck by falling object

IE was roof bolting when the drill steel hung in the top and pulled the chuck out of the drill pot. When the IE was placing the chuck back in the drill pot the steel fell from the roof hitting IE's hand. IE was checked out by a Dr.

EE was getting down from the loader at the end of shift. EE slipped off the last step, jammed left knee, felt & heard a pop.

Employee had walked outby in the track entry from the section to get miner bits that were laying beside the track. As EE returned walking inby to the section EE stepped on a wet rock causing EE to stumble and strike right foot on a rock which resulted in pain to EE's big toe.

while sitting in buddy seat of haul truck, a large rock went over the teeth of bucket and landed on the front right corner of the canopy causing employee to jar hurting EE's back

Employee was on the longwall working on getting shield out of the top and pulled up with the others shields, when the pan line pushed the relay bar struck employee's right knee causing laceration requiring sutures.

Employee was stepping off a Young Buggy from the operator's cab & felt a pop in right ankle. Seen site medic, referred to ER, X-ray's were negative, & a full work release was issued. Injury was still bothering employee, went to a physician at which time restrictions were issued for right ankle sprain on 11/5/2019 making injury reportable at this time.

Fall from machine

The truck had a leak at the hydraulic hose beneath the cab. The seat and an access cover were removed to gain access to the hose that ran on top of the transmission. EE was straddling the transmission to replace the base when they slipped and fell on their tailbone. Sustained a fracture to the tailbone.

Fall down stairs

Employee was walking down the stairs at the bulk bins when EE slipped and fell partially down the stairs. EE landed on back and arm causing pain in EE's back and side. Employee went to the doctor to be checked out.

Over-exertion in lifting objects

Miner was trying to lift a full tank of acetylene in to the bed of service truck when they strained lower back.

EE pressure washing coal truck on side of road as EE stepped around truck another vehicle passed and EE was hit by side mirror of passing vehicle. Vehicle did not stop. Hit right elbow and back.

Moving battery cell, pulled on it, got hernia.

Employee was setting up a car breaker when hand slipped off of breaker handle causing a sprain/strain to left hand. Employee continued to work until 11/12/2019 when employee went to doctor to have hand checked. Employee was removed from work by doctor on 11/12/2019.

The employee was exiting an end loader in Sugar Run Yard, when the employee stepped down the employee's right knee twisted. The employee was properly trained, and did not violate any laws or company polices.

I.E. was hanging miner cable from the top. In the process of doing this felt a sharp pain in groin. Employee continued to work until doctor restrictions on 12/17/2019.

Employee was removing unistrut (electrical strut channel) from roof in a building. Employee pinched right ring finger between unistrut and hand rail on scissor lift when EE was removing the unistrut. Employee noticed EE had smashed 1/2"" tip of right ring finger after removing glove. Employee went to hospital and received stitches.

EE was walking down a small surge pile when EE slipped on a loosened rock. EE reached out with the right hand to catch self and as a result broke thumb, sprained wrist and injured other fingers.

A fully trained miner was walking through a crosscut when miner tripped and fell injuring lower back. The injury was not the result of any compliance issues nor were any protective guards involved.

Worker states EE was gathering wear iron on the lower waste dump to stack onto wooden pallets. Worker states as EE attempted to move the fourth piece of wear iron it slipped out of EE's hands. The worker states EE attempted to catch the piece of iron with EE's left knee and felt a slight pinch in EE's lower back and pain in EE's knee.

The operator sustained head, rib and shoulder injuries from a fall into a railcar. The operator radioed that the operator needed medical attention but could not state location. The operator was transported to a local hospital by ambulance.

While lifting a panel door to gain access to the engine compartment, the hinge pin on the door broke off dislodging the door and causing employee to lose balance and fall down between the push arm and the cats.

Employee was operating a scoop when they complained of lower back pain. An MRI revealed degenerative conditions in lower back consistent with an MRI done in April of 2017. This is being reported as an occupational illness due to repeated trauma.

EE was walking through 2' mud from 777G to a pickup at end of shift-20' away. Right leg became stuck in mud, lost balance & fell on left side. Got up and went to pickup, felt pain in right leg. That night pain not bad. Next morning could not walk very well. Called Console Monday morning at 7 am to report accident. Contractor notified 10/30/19.

Employee was installing a rib bolt when a piece of rock fell out from the top of the rib and struck the employee in the chest and shoulder. The employee was taken off work for a separated shoulder.

Employee was moving material around shop, lifting idlers and felt pain in lower back.

Employee was between 6 West and 7 West when EE was struck in the head by a joint of 4"" discharge line, resulting in a contusion. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was salvaging a 282 power cable, while standing on a lift truck, as EE removed the last benita lip holding the plug & as EE started to lift it into the truck bed EE felt a strain to EE's lower back. Seen by site medic, referred to the ER, x-rays were negative on findings, employee was however restricted from work.

Struck against stationary object

While climbing down ladder, exiting loader, employee's foot slipped off 4th step causing employee to fall to the ground, striking head on ladder.

10/16 Office worker c/o back pain after walking up a stairway. Medical treatment refused by employee. 10/22 - Employee visited doctor and had clean x-rays, receiving no restrictions. Employee worked all shifts without issue until 10/25. 10/25 - PT restricted employee from climbing stairs.

Employee was sweeping the floor along North Mains belt. While walking backwards the employee stepped on a rock and sprained their left ankle.

Contact with hot objects or substances

Ee was starting EE's shift. EE put a lithium battery in EE's pants pocket and it came in contact with EE's keys, causing a fire. Ee burned both hands and upper right thigh. Transported to UofU burn treatment for care.

Over-exertion in wielding or throwing objects

Employee was repairing an aqua pipe when ratchet slipped off bolt causing them to lose balance and twist right knee resulting in a sprain/torn ACL. (Became a LTA starting 11-27-19)

A fully trained miner was attempting to pull cable slack with a loader. The miner instructed the loader operator to pull forward as miner was standing behind the machine. Miner had their arm to the left of the boom and when the machine moved forward it went to the right and the boom caught miner's arm between the boom and rib.

Injured employee stepped out of the operator's compartment of the diesel motor the injured employee was operating. Stepped onto the mine floor and rolled left ankle. Has pain and swelling in ankle. Employee did not violate any rules or regulations and has training and experience. Was wearing required PPE.

IE was helping carry a sheet of hog panel up to their roof bolter. They felt pain in left shoulder. IE continued to work up until 12/20/2019 when they went in for debridement of the left labrum.

Employee stated that while attempting to remove the pin from the belt structure, the belt rail fell and the bottom roller hanger struck EE on top of EE's left foot.

IE was pulling a hose on 11W belt when the hose got caught on structure. IE then pulled the hose with force and tripped. When IE finished the dusting setup EE noticed groin was hurting. IE was took to the Dr. and has not returned to work.

EE was working at the cone crusher and said that a rock had hit EE in the back of the head. EE was then transported to the ER by a company employee, IE was awake and talking the entire time just complaining of a head pains. It was determined that EE had a small skull fracture and small bleeding on the brain, EE also had a small cut on the head.

Employee was bolting up a round using the CMAC. Employee went to change the 6 foot steel when it slipped from EE's grasp and contacted right foot causing a contusion.

Employee was exiting a 20 ton motor when the safety chain caught employee's pant leg causing employee to trip and fall to the ground. The employee worked till 10/31/2019 when employee was taken off work for a torn bicep.

The employee was removing belt structure on reclaim #2 belt when a section of structure (Belt Rail/structure) hit the bottom bounced and shifted striking the employee on the top of the right foot just behind the toes resulting in fractures of proximal phalanges 2nd through 5th digits.

The employee went beyond a roller guard that was put there to prevent anyone from reaching the roller. They should not have done that.

The feeder bottomed out while tramming it to the new section. Cribs were placed under the rear end so the cats could be lifted. After the feeder became unstuck, the employee reached under the feeder to remove the cribs. The feeder teetered back pinching employee's finger between the crib and mine bottom.

The scraper behind EE hit hard while the two scrapers were linking up. EE had an onset of back pain over the next 5-10 minutes. Diagnosed as a ""Back contusion"".

A bundle of roof mesh was leaning against the rib, the miner positioned self between the mesh and the Roof Bolter. When the miner cut the bands on the mesh, the bundle of mesh rolled over pinning the miner against the Roof Bolter.

Employee was pinning in # 9 left entry. When top coal fell out between the coal rib and rib pin striking employee on the right leg twisting right knee. The twisting of the leg tore EE's ACL in EE's knee.

EE did not request medical attention at the time and finished shift. EE went on 10/18/19 to get x-rays.

EE stated that EE and another employee were attempting to hang continuous miner cable on the Unit 1 Section. In the process of the EE lifting the cable, EE slipped, and felt a pain in lower back.

While setting the feeder, EE was putting a crib block under feeder when one of the blocks crushed out letting the feeder set down on EE's hand.

As an employee was dismounting a haul truck, the employee's foot slipped off the bottom rung causing them to make contact with the ground. The employees hard hat fell off during the fall causing a minor abrasion to the back of their head. The employee was given medical treatment to clean the wound, and a prescription muscle relaxer and pain medication.

IE was carrying dust to a duster and walked into an I-Beam. IE claimed a headache and stiff neck so was taken to the doctor. IE was told to take some time off work.

Employee was pulling wire in conduit while standing on extension ladder in shop area when ladder slid down the wall causing them to fall injuring shins and right elbow. EE continued to work until 11-15-19 where they received an MRI and was schedule for surgery on 11-18-19 on right elbow and was taken off work.

Injured employee was in the process of installing roof support and was bending a roof bolt to install in the mine roof when they felt a pop in right shoulder. Employee was referred to a specialist for possible rotator cuff injury. Incident is being reported as a Lost Time incident.

Miner came in to the shop for a lunch break and was exiting the water wagon with EE's lunch cooler in EE's hand. EE's left foot slipped off the top step of the water wagon and fell to the ground, landing on EE's left shoulder.

Employee was a welding helper cutting and welding stainless steel. Employee worked a double shift and noted a problem on the drive home. Employee reported the eye incident the following day. Employee was taken off site for initial visit on 10/17/2019 and the following day was seen by an eye specialist on 10/18/2019.

The worker was working with our technician at the power cabin at the quarry. When our technician was not present in the cabin the worker did a non-authorized procedure in the power cabin and gets shocked. The skin on the worker's hands was burned.

IE was removing a steel cover over the foot shaft on a belt feeder. The plate fell and struck IE on top of the left hand. IE received 3 closed fractures to the left hand.

EE was roof bolting in the face of #6 entry when the incident occurred. EE was installing a 5' fully grouted bolt when hand & glove were caught on the rotating roof bolt resulting in a compound fracture to the left forearm. EE was rotating the drill chuck to align the chuck opening and the bolt head when the incident occurred. EE had surgery to repair the fractures.

EE was installing a roof bolt into the mine roof. While doing so, EE felt a pop in lower abdomen. EE worked the next shift, but the pain persisted, so EE went to a doctor the next day and was informed that EE had a hernia and has now been scheduled for surgery. Employee was assisting with flipping blow bars in a Stedman Grand Slam 6460 impact crusher. Employee got hand pinched between the rotor/blow bar and the sidewall of the crusher.

A crew was loading the chopper and a rock fell and landed on EE's foot. Another worker took EE to Georgetown ER, and to my knowledge was released same day and told EE could return to work in a week. I'm still working on getting paperwork and a statement from employee

EE was changing seat in a cat 777D rock truck. EE removed bolts from the seat, and lifted it to the ground using a crane. EE then took seat into shop with dolly cart. EE went to flip the seat over in the shop to remove the suspension from the seat body and felt discomfort in lower back.

The employee was repairing the bucket of the Loader Caterpillar 988G. To perform this work the employee was forcing an iron into the bucket using a hydraulic jack and a hammer screw. The screw detached from the Hydraulic jack and hit it on the head

Employee was walking to move a vent tube along the continuous miner, when a rock measuring 6' in length, 3' wide, and 18"" in thickness fell from the mine roof. The rock struck EE on the back causing fractures to EE's femur, ribs and vertebrae.

Injured was riding in a 14 man, mantrip. The mantrip door came open and as it shut EE's elbow was hit causing a small fracture. EE was seen at the Dr. and placed on restricted duty.

EE was holding onto the roof bolter cable and was pulling slack off while the roof bolter was moving from one entry to the next entry. While EE was holding it, EE felt a sharp pain in the groin. The pain continued over the weekend, so on Monday EE saw a doctor who has scheduled EE to have hernia surgery.

A portion of the coal / rock rib, measuring 8"" x 8"" x 18"", separated from the top of the rib / brow and struck the employee in the shoulder and neck area.

Employee was injured in a fall on a refueling platform on 10/10/2019. A recent medical exam determined the employee will need shoulder surgery and be off work until completely healed. Official verification that the surgery needed to mend the employee's injury was verified on 06/15/2020.

Employee stated that they were walking in the plant and their head struck a water valve. This caused pain in their head and neck.

On 11/8/2019 an accident report was filled out stating the IE shoulder has been hurting for the past month. After work comp/doctor visits. The IE believes the incident resulted from hanging a cable on 10/10/2019. It wasn't until 12/09/2019 the worker was taken off work pending an mri.

A fully trained miner was unloading a belt sled when the miner slipped and fell onto a piece of structure. The injury was not the result of any compliance issues nor were any protective guards involved.

Bypassed a guard, Failed to LOTO, and stuck hand in between rollers on the ABMO machine. These mechanical rollers caused a series of lacerations, abrasions, bruising, and second degree burns. The employee told no supervisor of the incident and put a glove on continuing to work. Claimed was hurt at home. Management became aware of the incident on October 15th.

Was rechaining crusher rotor during removal when the victim put weight on the rotor and it turned pinning left leg between rotor and frame.

The employee was moving the loader cable out of the haul road on the 7W Section in the 2 to 3 crosscut at 6 block, when the employee felt pain in lower back. It did not include any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was working on a crusher and climbed onto the crusher's drive belt guard without the required fall protection. Employee slipped and fell 6 feet onto the crusher platform. The fall resulted in a fracture to the pelvic bone, the employee was released with restrictions.

Absorption of radiations, caustics, toxic and noxious substances

Employee was working with pumpable cribs and was exposed to a mixture of concrete and water. Received burns on both legs just above the ankle.

Over-exertion in wielding or throwing objects

Employee backed miner out of cut, employee was throwing cable over and felt a pop in right shoulder. Employee went for medical attention the day of the incident with nothing found. Employee went to the Doctor the next day and could not return to work because of pain.

A fully trained miner was shutting the door on a scoop car when they caught a hand in the door. The injury was not the result of any compliance issues nor were any protective guards involved.

The employee was fixing a venting curtain on the 1WHG Section. When a piece of head coal fell from the mine roof and struck the employee in the back of the head. The employee was properly trained to perform the work.

After changing a deck screen, the Miner was crawling out from between the screen deck and misplaced their footing causing them to dislodge the board placed as a safeguard over an opening. The Miner fell through the opening and struck their head.

IE was carrying a 2"" air pump in TG-3 Air course. IE stepped/slipped in a rut falling to the ground and contacted the pump with IE's chest. IE sought medical treatment and a broken rib has been discovered requiring surgery. Mach HR was informed of this treatment on 03/04/2021.

On 10/8/19, the employee was descending excavator when EE's left knee buckled and EE fell to the ground. The employee was treated at the Company clinic where EE was diagnosed with a left knee sprain and a right shoulder strain, placed on restricted duty, and prescribed physical therapy

The employee was dragging a vent tube when they twisted their right knee. Employee was diagnosed with a sprained knee and taken off work on 10/9/2019.

IE was lifting a water prop to load on a car. When the IE picked up the water prop EE felt a pull on EE's left shoulder. IE was brought out and seen by a doctor.

The employee and 3 other coworkers were pulling miner cable out from under the slack pile. While doing so, employee felt a pain in right shoulder. After being examined by an orthopedic physician, employee was unable to return to work.

IE had walked to the back of the motor pit underground. IE then turned to walk back and tripped over a block in the walkway. IE landed striking left shoulder against the track rail. IE was brought out and sent to the hospital for treatment.

Miner was injured on 10/7/2019 and continued working until 12/18/2019 when miner's doctor put miner off of work. Miner was climbing down a ladder of a haul truck when miner stepped on a rock and hurt knee.

Employee was operating a scoop loaded with a LW shield. EE lowered the bill but failed to release the winch. This caused the shield tip to raise. The shield tip caught the scoop canopy, caused it to raise and slide off the side of the scoop. The light caught the employee's hardhat forcing their head down and backwards. No compliance, PPE or protective device issues.

Absorption of radiations, caustics, toxic and noxious substances

Employee was roof bolting, while putting resin glue cartridge in the bolt hole. it burst and some of the glue got in their right eye.

Employee was working at the river loadout on a barge and pulled on a crab line that was stuck. While pulling on the crab line, they over-exerted causing a sprain/strain to the left shoulder. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Repairing feeder breaker on surface. EE needed to remove pin to adjust bracket on hydraulic jack mounted to tracks. The pin in bracket was bound up. When EE air-arced through pin, EE thought that the jack and tracks would drop to ground. However, the tracks came up when the pin was cut. A different bracket on tracks struck EE in welding hood causing lacerations to forehead.

Employee was lifting a 5 gallon bucket of oil and EE felt a pop in EE's left arm. EE was diagnosed with a torn bicep muscle.

A miner reached into the head pulley area to dislodge a rock. The miner's right arm caught in the belt and pulled the miner up and over the top of the belt. A nearby miner immediately hit the E-stop. The supervisor was radioed immediately and cut the belt to release the miner. A tourniquet was placed on the miner's arm and an ambulance transported the miner to a nearby hospital.

Installing Reeving Ropes on dragline drag drum. Reeving rope secured to pro-locks on drum and was being lowered by crane from above via nylon rigging. Pace of drum rotation became greater that pace limit of crane causing nylon rigging to break and reeving rope to fall awkwardly below striking employee at right shoulder and left ankle.

The miner had asked for assistance with lifting a large chunk of concrete out of a cone crusher, but the loader operator they were working with was busy, so the miner decided to lift it out themselves. Back strain. Lost time accident as of 10/08/2019.

While activating the dump lever of the haul truck, the Miner received a spider bite to the lower left arm which began to swell due to an allergic reaction.

Employee was bolting on #7 Section and had the mast tilted to put pin in but couldn't reach to install pin, so EE climbed up and had EE's foot of the mast, let the mast down and caught EE's left foot between the mast and the drill pot causing a fracture to EE's left great (big) toe.

The employee was cleaning the chute to the crusher when a boulder rolled over on EE's right foot. EE's foot was bruised and swelled.

Employee was using a Lo-Trac to clean on the old E3-1 tailpiece when the Lo-Trac broke loose from the gob they were scooping and sent them into the chute catching them between the Lo-Trac and the the bottom part of the chute causing contusion/dislocated right shoulder and chest area complications.

Employee was struck in the head by a bottom stand that was lying in the haul road at 14-15 bk on 33 Butt intake. The stand flipped up and into the operator's deck of the scoop the employee was operating.

While using a hammer and chisel to knock out bolts after being torched, IE received a sliver of metal in the web of EE's hand between the thumb & index finger. Cause of this injury was awareness of this employee. Compliance with company policies & PPE may be a factor along with miner proficiency. Regulations/mining equip/systems, not a factor.

Employee was working on a flange to install a pipe. The employee used chain falls to lift pipe but pipe got caught on pillar and stairs in area. Employee pulled on the flange to get it unstuck.

EE was removing item from shelf when EE tripped and fell backwards onto the floor. EE hit head causing a small laceration. Evaluated in ED, mild concussion.

5 man crew was working in a wetland harvesting plants to be relocated to a different reclamation program. harvesting began at approximately 8:30 am, around 10:45 am a snake approximately 8-10 inches in length bit employee on middle finger between the second knuckle and hand. Snake was submersed in water around the base of the plant.

Employee connected the hook of the SRL to a positioning D-Ring on EE's harness the hook was not properly seated on the D-Ring, but rather the D-Ring was pinched between the tip of the hook and the hook gate (not seated in the hook throat).

While using a pipe wrench and attempting to remove a clean out plug on a continuous miner de-mister, knocked a rub rail off striking left shin. EE reported the incident as minor and insignificant 7 days later. EE sought medical attention 11/05/2019 where EE was placed 'off work' till further notice. EE broke a bone in foot after a rock from the crusher fell on it. EE went to close a side door on the crusher without properly shutting it down, first. EE was told by the plant manager to shut it down first, but EE did not. The plant manager saw EE approaching the machine while still running, and pressed the e-stop, but EE had already been hit by the rock.

When the driller reached the point where it tipped forward onto the ramp, the driller reached out and grabbed the door frame, the jolt released the door lock lever, slamming the door onto the driller's hand.

The employee raised the box to change the oil on a Polaris Ranger ATV. Once the oil change was complete, the employee was lowering the box by pushing on the top of the box when they heard a pop and felt pain in the shoulder.

Employee was recovering belt structure from an old belt line. While removing a bottom roller, they pulled on the roller handle and as the roller fell downward it struck a 4-inch water pipe. The employees middle & ring fingers on left hand were smashed between the handle and the pipe causing lacerations to both fingers and a bone fracture to the middle finger.

On 10/12/2019 EE was taken to the hospital where EE was diagnosed with two fractures in the left hand. While reinstalling the 980H bucket pin, that the EE had placed on the back of the bucket, the pin fell on left hand while EE was lowering the bucket, resulting in two fractures to left hand. *On 5/28/2020 the EE underwent surgery to remove fracture plate, resulting in 1 day lost.

Employee was cleaning out a brake box on the trailer of the haul wagon when employee's hand got pinched between the slack adjuster and the housing. The right index finger was dislocated and also had a laceration that needed stitched. The EE was wearing cut resistant gloves at the time of the incident.

The employee was not wearing a safety belt and lanyard while removing a screen from a portable screen plant. When the screen became wedged the employee crossed over the safety barrier to pull the screen. As it dislodged the employee lost balance and fell approximately 10"" to the ground. The employee sustained a fractured ankle.

Removing sheave from ISC motor-no space to fully lay motor parallel to the ground. The motor/frame were blocked from motion but sheave was hard to remove. EE was using impact wrench on sheave when the tapered bushing let loose & the wrench, sheave & EE's left hand were pulled approx 16"" downward. The wrench & sheave pinched the left pinky finger b/t the sheave & crusher structure.

The maintenance crew was installing new blow bars and rotor in the Universal 130/150. The rotor was out of balance due to the blow bars opposite of 5 & 6 were not yet installed. EE removed the blocking from the rotor, control was lost and the rotor rotated. EE tried to stop the rotor and got arm caught. This resulted in a laceration on each side of EE's forearm.

The employee was dismounting the grader after filling the coolant, and lost footing, falling backward off the grader, landing on back. The employee hit head and injured back in the fall. The manager was notified and medical was dispatched immediately.

The injured employee attempted to cross a supply car when EE rolled EE's right ankle on the car, causing EE to trip and fall. EE landed on the same ankle EE twisted on the supply car.

Employee is a Splitter Operator at our indoor Chilton Manufacturing Facility. Employee was handling two stones when EE pinched EE's left pinky/5th finger between the stones. The pinch caused an laceration to EE's left pinky/5th finger.

Employee was opening a sliding door on the polishing machine by pushing with one arm and pulling with the other. EE strained a muscle in EE's upper left back under EE's scapula. At 7:45 pm EE was loading the first truck of the shift. When EE went into the dig face for the second bucket, the bucket skipped off a boulder and jarred the loader whipping EE around in the cab. Initially EE didn't think much of it but as the pain turned into spasms that's when EE called to report it. EE reported this incident to me at 3:15 am.

EE received results from an MRI on November 26, 2019 and was diagnosed with a torn rotator cuff. EE is currently working full duty awaiting appointment with an Orthopedic specialist.

EE was changing auto lube canisters located on the pulley bearings. EE started to climb down, thinking EE was closer to the catwalk than EE was. EE let go of grip, foot slipped on mud and EE fell to the catwalk injuring EE's back.

Employee was driving haul truck in the 261 area when they accidentally engaged the E-stop on the truck causing the truck to come to an abrupt stop. As the truck stopped suddenly, the cinching style lap-belt came un-threaded from the male end of the buckle allowing the employee to strike the windshield of the truck.

While removing buckets from the stacker reclaimer with torches and hand tools a bucket fell off and landed on EE's foot. Bucket landed just above the steel toe of EE's right boot.

employee was loading a steel drill bit into the back of a truck. They propped the drill bit on the truck and placed hand down on the bed of the truck, while doing so the bit slipped off of where it was propped and landed on the employee's right index finger causing a contusion. The accident was reported and employee was sent to urgent care clinic to be seen by the doctor.

I.E. was dropping the pod after installing a roof bolt when the roof bolt wrench came out of the pod. As the I.E. tried to catch the wrench it came in contact with right ring finger in between the wrench and canopy post. Employee has not missed any work from injury.

A miner sustained a laceration and nondisp fx to the tip of the right long finger. The injured was preparing to replace a hydraulic hose on the WA 700 Front End Loader. While opening an access door EE states EE lost balance allowing the door to open fully catching the tip of the right long finger in a nip point between the cover EE was holding and another cover.

employee was hooking up a new 2 1/2 inch hose to a booster pump located at the plant. The hose was brand new and was raveled and kinked up prior to putting water pressure to it. The employee had a grip on the hose as the pressure increased it began to unwind, the hose caught the employee's right thumb and twisted it.

On Oct 15th EE was sweeping & shoveling on top of main vault roof. EE went home that night with no pain. At start of shift the next day EE reported to supervisor that EE was feeling a lot of back pain. EE diagnosed with Right thoracic back strain.

The EE was removing the drain plug from a 775F with a rachet. The plug was on too tight and would not loosen. The EE felt pain in shoulder as EE tried to break loose the plug.

Employee was attempting to climb a ladder to access a drain plug on a sand sluice pipe. While climbing the ladder the bottom kicked out causing the employee to fall approximately 5' to the ground, bruising left shoulder and left side rib area. Did not wait for a spotter to secure the bottom of the ladder.

Ee reports EE was inspecting heading and scaling when a rock struck EE's leg. EE reports left leg pain. Seen at the ER and clinic, released to modified duty, use crutches as needed. Diagnosed with abrasion to left thigh and contusion to left calf. Site was able to accommodate restrictions.

EE was walking across even ground and went to step up onto the truck scale (approx 12-18""). As EE began to lift self onto the platform, EE felt a pop in EE's left knee. This report is submitted under contest as operator is investigating the reported injury. On October 13, 2019, a miner reported stepping on a 1/2"" diameter water lance and feeling pain in EE's foot. Miner was wearing proper footwear and not performing any unsafe activities. X-ray showed a fracture to a metatarsal bone. Miner returned to work in a soft splint under restrictions.

Employee was scaling in a scaler when the cab lift cylinder bolt sheered off causing the cab to drop suddenly jarring employee in the cab. It fell approximately 13 inches where it hit a cab bumper block. The employee claimed to have a sore back and neck. The supervisor took the employee to the clinic and was diagnosed with a strained lower back and neck with restrictions.

EE1 opened clogged crusher, entered and began removing larger rocks through the crusher door. EE2 was on the platform outside moving rocks from the door over the edge of the platform to the ground. A rock shifted and pinched EE2 finger against the steel crusher door.

Employee was adjusting the forks on a forklift and they felt a pop in the chest area.

Employee was positioning cribbing and felt tightening of muscles in lower back. Evaluated by NP and given restrictions.

Ee was replacing a belly liner in the mill and the liner fell onto the thumb. The ee was seen, the wound was cleaned and stitched, and the ee was released to restricted duty.

Employee was installing roof bolts. The steel plugged, the employee struck the steel with the drill wrench. Upon striking the steel, the socket end of the wrench broke causing the socket to ricochet off the mast and struck the employee in the thumb.

While standing at the counter in the lunch room the employee turned and felt EE's knee pop. The employee called a few hours later to report the pain.

Loader hand found EE on the ground disoriented. EE asked for help and to go to the hospital. The ambulance was out on call so some of the other employees took EE to the ER. It was only after everything settled down that we found a rag and some window cleaner. Best we can tell EE got out to clean mirror or window and fell missing a step or handle.

Employee was completing pre-operating inspection when the engine hood spring mechanism failed. This resulted in the hood contacting the employees right arm. Employee was seen at the clinic where an x-ray revealed a fracture to the arm. Employee was released to light duty.

EE eyes were rinsed on site, but not all lime could be removed. EE was seen by a doctor to remove the remaining lime and given prescription eye drops.

EE reported on 10/6/2019 they had a sore back assumed to be from loading mill balls into ball mills on 10/5/2019. EE was evaluated on 10/6/2019 and put on modified duty for one day.

While working under the bucket of a wheel loader noticed that one of the braces against the cylinder of the bucket was unstable. While resolving this, one of the braces came loose and struck EE in the left eyebrow and shoulder, causing EE to fall back into a bolt rack. This caused bruising on EE's shoulder and lower right back with some bleeding from the left eyebrow.

Employee stepped on rocky area and rolled left ankle. Treatment was not sought until October 5, 2019 when diagnosed with a strained Achilles tendon. This became reportable on October 6, when employee began restricted duty.

An Matco Electric contractor was using a post pounder to drive rebar into the ground at the #4 shaft location. Upon lifting the post pounder up, the EE lifted to high causing the rebar to come outside the pounder. Employee was cutting off a bolt on a hatch on the inlet chute of the raw mill. There were 2 bolts in the door holding it in place. The one bolt that was holding it from rotating did not have a nut on it. As the employee began the work, the bolt without the nut somehow moved allowing the door to swing down and impact employee's leg, cutting it.

Changing out a belt chute on the 1st 54 South tail. EE had taken the bolts out to remove the chute. EE climbed into the tailpiece to move the chute. As EE was putting the sectional steels together with the bottom steel to the top, the steels did not come together properly. The outside person spilled a pallet of cement block while loading into a scoop tractor bucket with a loader. When loading block into bucket by hand they felt pain in their lower back. They continued to work the rest of the shift and went to a MedExpress the next day to be examined. They were diagnosed with lower back pain and prescribed a muscle relaxer.

Was wrapping dozer truck with chain and mechanic's truck crane. The track got stuck and the employee released tension on the track that is when the track inadvertently slipped pinching employees pinky finger.

Employee broke for lunch and deviated from the JSA compliant travel path. The path chosen by the employee had slip, trip fall hazards. EE as a result slipped and fell and struck the back of head on the concrete surface. EE blacked out temporarily and as a result was transported for medical evaluation which determined that employee had a mild concussion.

EE was in the process of changing the tail roller on C/M. While striking the connecting link of the chain with a hammer a piece of the connecting link chipped off and struck the employee in the left shin, embedding in the skin. EE was taken to a medical facility. The material was removed and the incision was closed with two sutures.

EE was using a cordless drill to drill a hole in a piece of rubber. While holding the rubber down with right hand, the drill slipped. The bit ripped through the glove and cut EE's middle finger on the right hand. EE was taken to the hospital and received stitches and medication.

The employee was struck in the cheek with a piece metal shrapnel while loading smaller pieces of scrap metal material into a lug box. EE used a JD350 excavator with a grapple to perform the task. A piece of the material broke off while EE was loading. The shrapnel went through the cab glass, striking the right cheek of the operator which resulted in 10 stitches.

The employee was standing next to the continuous miner in the #4 entry. The employee had placed left hand on the machine's light guard as employee was reaching for a can of oil, when a piece of roof rock, fell from between roof bolts, striking employee's left hand.

Employee was exiting a haul truck when they struck their knee on an open door and received stitches.

When the pump was started, the discharge line rotated on the clamp causing the pipe being used to strike EE on the right side of the forehead/brow, lacerating brow. EE was knocked back into the sidewall of the thickener. EE fell to the ground on left side. Co-worker approached EE & rolled EE to EE's back, keeping EE still, & called a mayday over the radio.

EE was cleaning cab of haul truck after night shift. EE wiped the seat and felt a pop and pain in their finger. EE was taken to clinic and diagnosed with ruptured tendon in finger.

Absorption of radiations, caustics, toxic and noxious substances

Employee clearing cement blockage got dust in eye.

An operator was walking to the break room. EE stepped in a hole (grass was grown over the hole) and EE rolled ankle. It was diagnosed as a fracture.

Struck by falling object

Employee was marking up #2 left entry, when wet coal fines that were stuck to the mine roof fell on EE's safety glasses & forehead. Employee took off the safety glasses to clean them, coal fines got into EE's right eye. Employee got a prescription for eye drops.

Struck against stationary object

Employee was loosening a hydraulic line on the skid steer. When the line broke loose employee hit their hands on an oil barrel that had been cut down for draining oil. The sharp edge of the barrel cut employee's hands between the thumb and the index finger on both hands.

Struck by flying object

Employee was operating a pit loader in the South Pit. While loading a haul truck, a rock came off the bucket & struck the cab/windshield. The windshield broke & sent small particles of glass into the operator's eye. The glass was not embedded in eyes. The particles were flushed out of the operator's eyes at the ER & operator was prescribed antibiotics as a preventative measure.

EE was using a utility knife to cut bucking rubber. As EE was shaving the bucking rubber which was on a steel sheet, the sheet moved and began to fall over. As EE went to grab the steel EE unintentionally cut hand with the utility knife. EE received 5 stitches on the inside of palm near the thumb.

Employee was using pliers to pull part when pliers slipped and struck employee over right eye

Struck by falling object

Employee checked the top before bolting and noticed a loose rock and tried to pull it. When the rock would not come down, EE drilled the hole for the bolt and when EE inserted the bolt into the hole and went to push the bolt up into the roof, the rock fell and hit EE's left pinky finger causing a laceration.

While helping to move the section, the employee got a finger pinched between the dolly and the scoop bucket as they were moving a cable, which required stitches.

Struck by falling object

Employee was struck by a rock while putting together drill steels before beginning to bolt off of the twin boom bolter. The rock fell from the roof and hit the ATRS canopy before striking the employee on the chin, causing a laceration that resulted in four stitches. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency not a factor.

Caught in, under or between a moving and a stationary object

The employee was using a 10"" X 1.5"" brass bar to bump a bearing race into a final drive hub. The employee unexpectedly missed the race, causing little finger to get pinched between the bar and race. The employee was wearing gloves at the time of incident. The employee fractured the tip of little finger.

Struck against a moving object

When employee connected 2 railcars together, the force of the connection caused employee's head to jolt forward and strike mouth on frame of open window causing a laceration to the upper lip which required stitches.

Struck by falling object

EE was working on a continuous miner. Had raised a lid up to examine a proximity device and tied the lid to a beam above the miner. When in the process of lifting a second lid, the lid bumped the first lid and caused it to fall landing on their left index finger. This resulted in a laceration around the finger and had to have stitches to close the wound.

A mechanic cut finger while changing belt splice. EE was holding the back of the clip with the other hand to apply pressure to the back of the clip, the bolt came free and contacted EE's right middle finger resulting in a laceration, requiring 4 stitches.

Drilling holes to bolt guards into place, steel piece got into right eye, employee said was wearing safety glasses, no witnesses.

Employee was holding a drift pin when a piece of the pin broke off and caused laceration on leg.

EE was exiting EE's dozer. It was raining and EE's top deck was covered in mud and slick. As EE went to step down EE's foot slipped and EE grabbed the door to EE's dozer. The door slammed shut on EE's right hand and pinched the tip of EE's right middle finger between the door and door frame. This resulted in a laceration. EE received 9 stitches to close the wound.

Employee was using angle grinder with safety glass on, piece of metal found its way into eye; employee came back to work the next work day (week day crew). (misreported as first aid case, MSHA instructed operator to re-submit 7000-1 for this case).

Contact with hot objects or substances

The EE was using a cutting torch while working on a S/C conveyor chain. The torch slipped out of EE's hand. As EE went to pick it up EE's knee contacted the hose and spun the torch head causing it to go across EE's right thumb.

Miner was cutting rubber belting material with a razor blade knife. The knife slipped and the miner lacerated left forearm with the blade of the knife. The laceration required sutures and the miner returned to work.

EE WAS TRAVELING OUT OF THE MINE FOREIGN MATERIAL FELL FROM THE TOP HITTING THEM IN THE FACE AND EYES

Employee was opening a port on preheater tower cyclone, when a piece of coating fell and forced some material to come out of port. Some of this dust got into employees right eye.

Employee walked out of building after light rain and foot slipped on concrete rampway exit causing a turn to grab right side handrail preventing fall. The right knee and right shoulder were stiff and sore the next day. Employee provided first aid and diagnostic X-ray/MRI negative on knee but provided cortisone shot on 12/19/19 follow-up.

Employee was adjusting the width on the forks of the Mini-Trac. EE was attempting to slide the fork over when they slightly picked up on the fork and it slide abruptly. Their finger was caught between the mast and fork. The tip of the left middle finger had a laceration that took 4 sutures to close.
