Accident type, without injuries

A roof fall was discovered at East 1-60 xcut between the track and belt. The fall was 20' long X 15' wide X 7' high. The fall was called into the MSHA hotline at 3:53 pm. The fall was investigated by MSHA Inspector on a later date. Weathering was a contributing factor.

An unplanned Roof Fall occurred in Main West Intake XC #19.5 (#2 Entry - Primary Escapeway). The cave is approximately 10 by 10 and 4 high. No one was injured or entrapped. Preliminary diagnoses; 2 supplemental supports in the form of Steel Sets and Beams were unknowingly hit by a trailer, causing steel sets to fall over. This caused the roof support to fail.

A ROOF FALL OCCURRED ON THE 1ST SOUTH MAIN BELT AT SPAD 21+70 (x-cut 35). THE FALL WAS 55X18X10 ON THE BELT. THERE WERE NO INJURIES DUE TO THE FALL.

A hot spot was observed on the raw coal storage pile. No one was injured. The material has been being moved by a loader onto trucks to be hauled away.

Miners called investigator and 911 after discovering the fire in our shop. The cause of the fire is still under investigation. Approximately 40 feet of the southeast corner of the roof and south wall sustained burn damage. No personnel had been in the shop since 3pm.

An unintentional roof fall occurred in the b mains section MMU 001. The fall started in an unsupported cut in the crosscut 3-2, 1 crosscut outby SS 5338, and extended outby for approx. 30'. Width of the cave was approx. 18' and the height was undetermined. Exact cause of the fall is unknown but we assume that reduced cover and a stream channel contributed to the fall.

An unplanned Roof Fall occurred - 1st West Pillar Section between XC 17 & 18 - #9 Entry, inby the loading point. Approximately 17 by 20 and 4.5 high. No one was injured or entrapped and no ventilation was interrupted. Preliminary diagnoses; Cable Bolts were being installed to replace Floor to Roof Supports. When 2 timbers were knocked out with the ATRS the fall occurred.

An unintentional roof fall occurred in 3 East Mains #1 Entry 7 Wall intersection. There were no injuries or damaged equipment as a result of this incident. Conditions contributing to the roof fall are adverse geological conditions in the stack rock layer and horizonal stress in the mine roof.

Airway examiner discovered roof fall at spad 15409 adjacent to the No. 5 Belt at 18 CC in the no. 10 entry. Fall measured approximately 75 feet long x 20 feet wide x Unknown height (pulled anchorage).

1C panel 002 MMU BK33 8R second cut fell in after miner finished mining. When the top fell it road back through the last row with deep cut bolts in it.

The SP Material Hoist was taken out of service due to a spot being found in the rope. The area in question was too far from the car to shorten the rope. The rope was replaced with a new rope. The hoist was tested and placed back into service. MSHA was notified via the hotline and an email was sent to the FOS.

A roof fall above anchorage occurred on 09/24/2021 in J-entry #2 and #3 entries between XC 9-11.

A contractor was stuck in the Clinker Silo elevator for more than 30 minutes on September 23rd, at 9:30am. A report was not submitted, nor anyone notified at the time of the event. Statements were obtained on September 24th, at 3:00pm. Contractor was safely removed from the elevator without injury, returned to work immediately.

An unintentional roof fall occurred in the G2 Longwall inby 11 crosscut #3 entry, approximate spad 122+10.52. The fall was approximately 15' Long X 8.5' Wide X 16.5' height. The immediate roof consisted of laminated shale type material. Adverse geological conditions contributed to the failure. The fall was just at the longwall face at the time the fall occurred.

In muck bay #2 located in 4041 Barrel, shotcrete support came off the right rib while mucking the face. The measurement estimated to be 47' in length. The back & left rib were still supported. No injury occurred nor damage to equipment. Due to shotcrete separation, based on some corroded bolts & air slack, the muck bay couldn't be used till a plan was developed to rehab the area

An unplanned Roof Fall occurred in the 1st West Pillar Section, in the #5 Entry - Intersection of crosscut #19 (Pillar Line). The fall is 20'x20'x 8' high. No miners were injured or entrapped. No ventilation was interrupted. 4-MRS units are inby the fall. Preliminary diagnoses; The Intersection was developed too wide which caused the strata to fail.

An unplanned roof fall was found in the left return on SC panel No.2 in the intersection outby Survey Station No. 1251. Area was dangered and cribbed off.

An unplanned roof fall was found while traveling the right return to check EP and MP points. Fall was located between survey stations 1351 and 1352 at mouth of SC panel 4. Area was dangered and cribbed off.

A roof fall occurred on the 3W/3N panel at 10xcut (660') in entry #3. The fall measured 20'Wx20'Lx5-6'Th. No one was injured and ventilation was not disturbed.

Two outside contractors were using the elevator to access the top of the 314 bucket elevator. At 4:45 PM they were taking the ""Silver Bullet"" elevator down to get some tools and it stopped between floors. Otis repair was called at that time. Otis arrived on site at 6:22 PM and the two men were released at approximately 6:45 PM.

Top weakened and fell on the 9RT Longwall belt. Fall was 25 foot long X 19 foot wide, and 7 to 11 foot high.

There was a roof fall at 69 crosscut on Main North 3 in the right return. This was in an air course and did not impede travel or ventilation. The route can still be made in its entirety. The 3 approaches to the fall have been supported.

On 9/15/2021 a roof fall occurred in the No. 3 beltline entry at x-cut No. 56 on the DS-1 mains. The roof fall is confined to the No. 3 entry. The roof fall is approximately 30' in length X 19' wide and approximately 8' thick or high.

A roof fall was found at the river portal on the mainline haulage at 48+00. The fall was approximately 50 feet in length by 16 feet wide and varied from 8 to 10 feet in height.

Found an unintentional roof fall on the #3 Belt inby xcut #25 and extending to xcut 26.

#3 Hoist computer screens went down. Operator tried to fix issue. Management was notified and Dispatch called MSHA. Issue was fixed by resetting computer. #3 Hoist is a skip hoist. No person was entrapped or injured and no damage to hoist or shaft. Computer issue. MSHA was called back when issue was fixed.

An unexpected roof fall occurred on the 2W Main in entry #3 (primary escapeway) at cross-cut #5. The fall measured approximately 18'x 18'x 6+' in thickness.

Unplanned roof fall between crosscut #14 & #15 #1 entry of the Main North #2 return inby spad station #2387.5. Supported with 7' TR Bolts and 12' cable bolts. Fall dimensions - 17' wide, 20' long, 7' high. Fall did not cause injury, impair ventilation or entrap miners. Fall area developed on 1/8/2019. Reported to MSHA at 6:47 AM on 09/08/2021.

An unplanned roof fall was discovered at 1 North 34 block crosscut from the track to the belt. The fall was approximately 35'L x 16' W x 10'H. No one was injured during the fall.

A roof fall had occurred on Number 10 Belt in the Belt Entry at Cross Cut #41. The fall was above anchorage and was 15 FT long, 18 FT wide and approximately 12 FT high.

The Pack house elevator stopped between floors just as it was lowering two employees down. Maintenance attempted to reset the unit but were not sucessful, Thyssenkrupp elevator was called out to lower the elevator. Two employees were in the elevator for longer than 30 minutes and the event was called in. Non injury

Unknown at this time. Alternative (Alt) fuel supply ignited within the Alt fuel baghouse, damaging the baghouse internals and feeder belt within the equipment.

A fire occurred at the Enfield Bell Mine ID. A surface water truck was dispatched to help put the fire out. The MSHA hotline # was called at 1:42am to report the fire. The fire was reported to be out at 2:06am. The cause of the fire is currently under investigation. No damage to structures or injury to personnel occurred.

Tailgate egress is blocked. Tailgate blockage plan has been implemented. Last shield on tailgate is inside the coal block 4-6 feet making it difficult to ""hole out"". Excessive rock is sloughing into the TG-4 #4 entry impeding egress off the TG side of the LW Face.

After cooler Cyclone Dust Box experienced a structural failure, as an apparent result of a rapid expansion of the internal atmoshere.

An unplanned roof fall at or above anchorage occurred on the South Mains #1 belt at 14 x-cut. The fall measured 10'L x 16'W x 6'H.

Approx. 4:55a.m. in West A Mains between 4 and 5 Room at 6 XC, bolter operator was bolting top. Bolt sparked off pan and start fire from methane venting off floor. Immediately, bolter operator stopped machine and extinguished fire with muck and fire extinguisher. MSHA was called and MSHA Inspector investigated event.

A non-injury roof fall occurred on Northeast Mains, #3 entry, Break #26. The fall was isolated to the intersection of #3 entry intersection extending approx. 8' into the left xcut toward #2. The entries to the fall area are dangered off and supported. The lifeline has been re-established in the #4 entry and a cleanup plan has been developed to clean up the roof fall.

A roof fall was discovered in the track entry adjacent to the No. 3 Conveyor Belt, near the No. 8 cross-cut at spad no. 445. Fall was approximately 20' long x 20' wide x 5.5' high. No injuries reported.

An unintentional roof fall occurred in the 9L section. Adverse geological conditions in the form of layer shell rock separated causing this occurrence. The fall occurred at 12+00 on the future longwall face. The fall measured approximately 30ft long, 16ft wide 20ft high. No injuries or equipment damaged as a result of this incident.

An unplanned roof fall was discovered in ML #4 Belt at 36 block. The fall was approximately 60'L x 6'H. No one was injured during the fall.

Caught in, under or between a moving and a stationary object

For unknown reason an independent driver lost control of belly truck which caused it to jack knife, throwning driver from the cab. Driver was then run over by same truck.

Unclassified, insufficient data

Weld crew initiated an argon purge on a 30"" pipe that had been fit up afternoon prior. When argon purge was established, two welders began to make their root pass welds. Upon inspection ""sugaring"" was observed. a welder entered the open end of the 30"" pipe for unknown reasons where they were found by a coworker to be unresponsive. No witnesses observed them enter the pipe.

A visitor to the mine site was in a unauthorized area running a sluice box in a unstable trench when it collapsed and buried them. The trench was tapered and made safe to remove their body.

Caught in, under or between a moving and a stationary object

Truck operator was clearing materials off from the tail gate with rt hand when the latch mechanism pulled the gate inward pinching the tips of operator's fingers & glove. Operator was taken to ER where they sutured the distal tips of right middle & ring fingers back in place. On Sep 27 a hand specialist determined that right middle & ring fingers distal needs to be amputated.

Employee was working near the S-drive belt on the screen feeder. Hand got caught in the belt.

Over-exertion in lifting objects

EE went to lift a large tote of grease. EE experienced no pain at the time of the lift. On 9/7/21 the employee reported to supervisor that they had a protrusion near belly button. The employee was taken to the clinic and diagnosed with an umbilical hernia. EE is on restrictions awaiting further care.

Caught in, under or between a moving and a stationary object

While installing a metal collection box via use of a forklift to position in place and a ratchet strap being used to align bolt holes, employee placed hand on flange of metal box to brace self while releasing the ratchet strap. As the collection box made contact with the existing structure, the employees index finger became caught between the bolt flanges causing injury.

Fall to the walkway or working surface

Ee finished cleaning walkway, rolled up hose. Walked across walkway to get other hose to continue cleanup, slipped in mud on the side of the walkway, both feet got stuck, ee fell to ground hitting head and shoulder. Seen at ER, laceration to forehead closed with stitches, ee taken to surgery to repair fractured shoulder. Put off work until 10/15 follow-up.

IE was at the NEM Winch installation site when the NEM Belt was started. IE reached up to touch the bottom belt (for an undetermined reason). IE's right hand was pulled into the bottom roller and caught between the bottom belt and bottom roller. EE attempted to free self with EE's left hand and it too was pulled into the roller as well.

Struck by falling object

EE was working on mobile camera. While cranking up the camera mast, the trailer jack stand failed causing the front of the trailer to fall to the ground. When the front of the trailer collapsed, the camera mast dropped striking EE's right ring finger cutting the tip of the finger. Stitches were required to close the cut.

The employee was removing the bucket attachment from the Gehl Skid Steer, and when EE released the attachment keeper pin, the bucket fell off of the attachment plate and slid into the the right knee. Resulting in a contusion to EE's knee.

Over-exertion (Not Elsewhere Classified)

Performing an inspection inside of a tank, coming out of the tank from the side opening, feet first and had pressure to EE's biceps pulling self through the opening, lost grip with EE's right hand, used EE's left hand and arm to catch self and immediately felt pain and burning sensation in EE's left bicep.

Employee was driving a spad into the coal rib and got a piece of coal in right eye. The eye was scratched.

Contractor stated that they raised the lid on scoop batteries and when they went to let them down, they wouldn't close. Another employee raised the lid back up and when EE did EE stuck EE's hand in to check latch. The lid slipped out of the other employees hand and fell on EE's left hand resulting in fractures to EE's left index, middle and ring fingers.

Struck by falling object

A fully trained miner was working on a motor when the lid of the motor fell on EE's back. The injury was not the result of any compliance issues nor were any protective guards involved. NOTE: Employee did not begin missing work until 10/01/2021.

Contact with hot objects or substances

The employee indicated EE was welding the bucket and did not realize that the cable cover was defective and having contact with the bucket created a spark and caused a small burn on right leg. EE was checked, cured and indicated that EE could continue working. EE continued work normally until 10-6-21 which indicates that EE is uncomfortable with the area.

Struck by falling object

The employee was stacking blocks to build a stopping when several blocks fell from the top layers striking EE's left hand resulting in a laceration to the palm requiring three stitches and a fracture of the left thumb.

Contact with hot objects or substances

Employee was welding in the box chute off #1 screen when a piece of hot slag fell on EE's pant leg and caught pants on fire causing a burn to lower leg.

Over-exertion in lifting objects

Employee bent over to re-position a 72"" piece of belt structure for loading into a scoop when they felt pain in back. Employee was diagnosed with a protruding disc and placed off work on 11/19/2021.

Caught in, under or between a moving and a stationary object

While taking down scaffold, injured employee had elbow stuck between runner and brace they were taking down.

Struck against a moving object

Employee was parking EE's mantrip on the 7 North Portal Bottom, 11 blk. spur. As EE entered the spur track, EE pulled on EE's brake lever and locked up EE's brakes and slid into a parked mantrip. Employee fell from EE's seat and landed on EE's back inside the mantrip.

Fall onto or against objects

EE was exiting a screen deck when EE stood on the door that had become wet from light rain which made it slippery and EE's feet slipped causing EE to fall straight on butt causing severe back pain. The EE had called another EE to come help finish the job but EE had fallen before the second EE got there. Has a t12 and rib fracture.

Fall from machine

Haul truck driver was dumping a load into the apron feeder of the crushing plant. They finished dumping the load and heard a loud pop when the bed of the truck was fully in the down position. Driver then saw smoke and flames. The operator got out of the cab went down the initial set of steps and then jumped off of the lower set of steps onto the pit floor and broke an ankle.

Over-exertion in lifting objects

Miner says EE was just cleaning up around the guillotine slab and when EE picked up a rock EE felt a ""twinge"" in EE's lower back. By the time EE drove home EE was in pain and went to hospital. EE didn't report to anyone until the next morning. EE had x-rays and was given a prescription for ibuprofen and a 7 day excused from work note.

Trucks were using a back road to access the truck dump due to the tracks being blocked by a train at the crossing. EE operating truck 807 proceeded around a horseshoe curve hauling clean coal from Eckman Plant to Dans Branch Load-out. The driver stated EE misjudged the curve and ran up on the berm due to poor visibility causing the truck to roll over.

Over-exertion in lifting objects

Employee stated EE was cleaning out the Scoop Deck and a sharp pain occurred in the lower center of EE's back.

Struck by falling object

While drilling the roof in No. 7 Right on the No. 1 Section, a piece of rock came down the drill steel striking employee in the nose.

Fall to the walkway or working surface

EE was in the parts yard looking for a replacement hose and stepped on a parts pallet which broke causing EE to stumble and fall.

Caught in, under or between a moving and a stationary object

Miner trammed rail mounted locomotive onto foot, pinching foot between frame of locomotive and ground.

Fall from machine

Employee lost balance and fell to the ground while dismounting dozer. Employee suffered a fractured left heel.

Over-exertion in wielding or throwing objects

Employee was shoveling on the belt line at 8-42 when they lifted the shovel and felt a pain in lower back. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was rough loaded while in operation of haul truck, was given restrictions at a medical follow up on 9-29-2021

Struck by falling object

A miner was bolting the back of the 833 N stope. A fall of ground in the hanging wall fault zone buried the miner until the miner could be rescued by other miners on the crew. The miner received a broken jaw, broken femur, broken rib, fractured hip, several face abrasions, as well as missing teeth.

Over-exertion (Not Elsewhere Classified)

Employee was standing on rocks beside the miner when one of the rocks broke, causing the employee to roll EE's left ankle. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Fall down stairs

Miner was walking down a set of stairs and slipped on the third step from the bottom of the stairway, falling to the bottom and straining EE's back.

Over-exertion (Not Elsewhere Classified)

Employee stated they were walking from #6 entry to #7 entry to hang fiber cable, when they stepped in a hole that was approx. 10 to 12 inches deep, and felt a pain in left knee, Employee started missing work on 10/6/21 when they went to the doctor, we do know they have had problems in the past, not knowing if this needed to go on a old claim is why we are late filing it.

Fall to the walkway or working surface

Employee was sitting on the back of the mechanic truck and stepped down and started to turn around when EE's ankle gave causing EE to lose EE's balance and fall to the ground. EE tried to catch self with EE's right arm causing EE's injury.

Injured employee was moving C/M cable out of the roadway when a rock broke loose from the mine roof. As EE turned to run away, employee felt pain in the right knee. The operator received the results of an MRI on 11/8/2021 which reported a fibular ligament tear. This is now being reported as a Lost Time Incident.

Over-exertion (Not Elsewhere Classified)

IE was walking from the #3 Entry face and tripped over some gob. IE twisted left knee. IE continued to work until 10/4/2021. Removed from work by medical on 10/5/2021 pending MRI. Safety dept notified on 10/11/2021 of this action.

Struck by falling object

Employee was mining the face of #12 entry at spad 16+50 on the #1 unit when a piece of rock 2'x8'x3"" fell between the rib pin and rib striking EE in the left ribs and back. There were no signs of loose rock or any other hazards in the #12 entry.

Struck against a moving object

EE changed the rear diff fluid in water truck at shop. EE checked the brakes and low pressure light in the cab to make sure they had pressure. On the way back to the pit, EE stopped several times and knew they had brakes. At some point on the pit road, EE was unable to stop and wrecked the truck at the bottom. EE was fine, but the doctor kept EE out on Monday.

A fully trained miner stepped off a raised area into a hole and jarred EE's back. The injury was not the result of any compliance issues nor were any protective guards involved. NOTE: Employee did not begin missing work until 9/25/2021.

Employee was loading machine and felt a sharp pain in back and right leg

Struck by falling object

Employee was rib bolting in the 4B miner section when EE swung the rib bolter towards the rib, the bolter hit a loose bolt knocking over a stack of roof plates that fell from above the rib bolter controls striking the employee on the left forearm causing a laceration. EE received fifteen sutures.

Contact with hot objects or substances

Operator cleaning out discharge chute on rotary drier when operator stumbled and right arm went into wheelbarrow of hot material filling glove and burning rt. hand. First aid administered at plant, transported to ER and later admitted into hospital.

Struck by falling object

During a power move on the longwall, an employee started dropping highline at 49 BLK and the employee injured walked out of 45 BLK crosscut. Injured employee was struck on the right shoulder and back with the highline resulting in multiple fractures.

Fall to the walkway or working surface

Tightening a nut with a pipe wrench when the wrench broke causing employee to fall back onto catwalk injuring arm and hand.

Struck by powered moving object

Employee was bent down cutting a piece of belt in a cross cut behind a diesel mantrip. A diesel scoop turned into the crosscut striking the employee on left torso. Employee sought medical treatment on 09/23 and was released to come back to work. Employee went to doctor again and was placed off until 10/4 where results show contusions to rib area.

Over-exertion in lifting objects

I.E. was helping hang continuous miner cable across the last open crosscut. While hanging the cable I.E. felt a burning feeling in right shoulder. I.E. has continued to work until missing work on 9-30-2021 due to the injury.

Caught in, under or between a moving and a stationary object

A fully trained miner was profiling belt structure when a finger was smashed by the belt roller. The injury was not the result of any compliance issues nor were any protective guards involved.

Struck by falling object

Employee was tramming scoop when a piece of rib sloughed off, falling into the deck of the scoop striking the employee's small finger ( Pinky finger ) causing laceration.

Over-exertion in lifting objects

Employee experienced some pain in their back and right arm as EE was helping hang miner cable. This incident was reported on 09/22/2021, employee continued to work until 10/12/2021 when EE was taken off work by their doctor for treatment.

PULLING CORD ON OUTBOARD MOTOR TO START IT.

Bodily reaction, (Not Elsewhere Classified)

The employee was doing routine maintenance on a roof bolter and when EE stepped over a grease hose felt pain in legs, lower back, and knees. EE was working at ground level and states: raised left foot about four to six inches to make a step over a grease hose. When EE put left foot on the mine floor after making the step is when the pain started.

Employee stepped down off the relay bar and felt a pain in left knee. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

EE was loading supplies on the roof bolter. When EE twisted EE felt pain in EE's lower back. Returned to work on 9/27/2021 and worked until 10/02/2021. EE is awaiting further medical advice.

Operating a cutting torch while laying on side. A spark fell in miners ear canal.

Employee was adjusting a 425 Symonds crusher using a come-along adjusting bolts, was not loose. Employee ""side loaded"" come-along breaking connection to hook, causing chain to hit EE in EE's side/back.

A fully trained miner was moving an air drill when EE felt pain in EE's lower back. The injury was not the result of any compliance issues nor were any protective guards involved.

On 09/22/21 IE was roof bolting on Unit 2. While installing the 3rd row of roof bolts, IE swung the drill boom towards the right rib. The IE set the drill boom. It is believed that the foot of the drill boom was either positioned on a rock or weak floor material. The base smashed the rock/weak floor and lowered the drill boom, striking the IE in the hard hat.

The employee alleges EE was driving the loader across the pit and EE ran over a large rock causing the seat in the loader to bottom out. EE started having some pain in the middle and lower of EE's back.

Climbing extension ladder to get platform on front of crusher to check main belt take-up adjustment. Slipped and/or lost footing and fell about 8' on back.

A miner was injured when they were handling a belt structure Top Roller. Miner tripped & fell to the mine floor. The roller they were carrying struck them on the left shoulder as they fell. Miner sustained a deep bruise to the left shoulder.

Employee's feet went out from under them when they stepped on a smooth surfaced rock that was wet from sweating, and they landed on right knee and it folded up under them. The employee was bolting with an air bolter on 2 ML Belt. When the injured went to put the drill steel in the hole and shove up, EE felt a pop in the left shoulder. The injury did not become reportable until EE being given off work on 9/21/21.

Employee was carrying a drill down #6 belt line when EE claims EE felt a ""pop"" in left heel. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

Employee was working on crusher chute. The chute has a door which opens and closes for various stone sizes to be directed to. Employee got a pinky finger caught in one of the doors. (Doors are sometimes referred to as gates).

At approximately 9:00 AM an employee opened a butterfly valve to fill the water truck. When the truck was filled with water, the employee closed the butterfly valve and alleges that EE hurt back while doing so. Employee left site and visited their personal chiropractor who took EE off of work.

Employee was preparing to clean up work area with water hose when EE slipped on muddy water and fell. EE tried catching self by putting down right hand. In doing so, EE sprained right hand and wrist. EE has been placed on restriction/light duty until further notice.

Employee was standing on the first rung of a ladder while repairing/replacing a guard. Employee slipped off the ladder and landed on a knee. The knee made contact with the concrete pad. EE visited a physician on 09/23/21 and returned to work on light duty. On 09/30/21, the clinic notified us the EE is to be withheld from work pending the results of the MRI.

Employee was emptying out a railcar loaded with Gypsum rock. EE was removing an air powered vibrator (weighing approx. 45 lbs.) from the railcar. EE removed the vibrator and turned to lay it down on the ground, approximately 36 inches. When EE turned, EE felt a sharp pain in back.

IE was prepping to remove a tail drive cover. IE picked up a piece of metal to toss to the side. IE struck hand against a piece of rack bar. Right ring finger was caught between the pieces of metal resulting in a fracture and laceration of right ring finger.

Ee was seen at ER, CT negative for acute injury, ee released to full duty. Ee complained of ongoing symptoms that were determined occupational on 10/4/21, and put off work, prompting this report.

Miner was climbing into haul truck when EE lost EE's footing. Miner fell to ground landing on feet. This resulted in a fracture to left ankle.

While an employee stepped down from the emulsion truck, EE's foot slipped on a rock in the underground mine. Employee was seen by doctor and treated for bad sprain and/or possible avulsion fracture.

Employee was positioned on #25 shield when the shield began to lower and pull in. The injured was caught between the hoses on the shield and the pan line. The employee was examined at a hospital and diagnosed with a fractured pelvis.

While hand-wheeling a 380 lb. tractor drive wheel into position to be changed, a haul truck driver lost control of the wheel and then fell onto the wheel, resulting in 2 broken vertebrae in EE's back.

IE was working on the surface standing by a stack of drill rods watching the drill rig, a end-loader drove by the other end of the stack of drill rods and caught one of the drill rods spun the rod and struck IE in the back of the legs. IE x-rays were negative and is still under doctor care and receiving therapy.

The employee was riding in on a man-trip and hit a pot hole. EE was bounced up and when EE came back down EE struck EE's middle back on the seat. EE then continued to work until the pain got worse. EE was then transported to the hospital.

Employee was using a cutting torch when the torch started to back burn into the barrel. The employee removed the acetylene hose and attempted to clear the torch barrel with oxygen. The mixture inside the barrel was still lit and shot a fireball down the inside of the left glove. This gave the employee 2nd degree and 1st degree burns on the inside of their left palm.

Employee was using Lull forklift to cover bag load from rain damage. Boom should not have been used to perform task. Boom was all the way out while traveling, forklift started leaning. Trying to balance the forklift, lever got stuck & tipped on side. Employee will be retrained on using the proper equipment to perform the task.

Employee was standing next to the can setter controls on the scoop with leg bent and foot against the rib causing a knee to stick out. Employee was injured when scoop bucket swung towards the rib causing injury to the knee.

Employee was changing out agitator paddles, one fell & struck EE in the back of right hand. EE's hand was in midair, not on any surface. EE noticed mild pain at the time & noticed severe pain, swelling & decreased ability to move fingers at about 0300 on 9/3/21. EE was seen at onsite clinic and referred to ortho. Restricted use, no work. Right hand in rigid splint.

EE was helping to load a trailing cable on a mule. EE stepped over the tongue of a lowboy & EE's foot got caught in the safety cable, causing the EE to fall forward landing on the left shoulder. EE continued to work w/o limitations until placed off work by an orthopedic surgeon on 2/9/2022.

Employee received a scrape on right calf area. Employee initially refused medical evaluation. The scrape became infected which resulted in medical treatment which resulted in 3 days lost time.

Miner was operating the CAT 349F excavator approx 25-30' from an approx 42' highwall when it sloughed causing material to cave-in the bottom and top windshield allowing material to enter the cab trapping miner for <30mins. Miner made distress radio call and miners immediately began removing miner from cab. MSHA was notified <15mins. Miner received medical care on 10/4.

Employee was acting as a spotter for 6""x400' pipe removal by excavator. During pipe movement the excavator changed directions causing the pipe to slide 20' unexpectedly in the direction of the employee, striking the employee's left leg, fracturing the tibia and fibula.

Tertiary air duct was being cleaned. Miner placed a jackhammer bit against equipment and stepped away to cool off. Miner returned to close the door to tertiary and bumped the jackhammer bit knocking it over, striking an air cannon. The air Cannon discharged and blew material out of tertiary, striking the miner. Miner was burnt on Chest, Neck, and Left Arm by flying debris.

Apx. 760' HPDE pipe was being pulled down an access road. Around a 100 degree bend, the pipe developed resistance from unidentified debris (presumed rock). The resistance gave way and the tension applied from pulling the pipe caused it to straighten striking and employee inspecting for holes.

Citation No. 9534976 was issued during a Part 50 Audit with regard to this injury. The employee was pulling on a drill steel that was stuck in the mine roof when employee felt a pain in lower back.

Belt was lowered so chains could be adjusted. While lifting the operator also went into reverse & one unhooked, catching the frame throwing a miner to the ground. This miner received stitches for a laceration to right side of face, two broken bones in left wrist, and three broken ribs on the right side of rib cage.

Employee was working on a belt move and and belt structure came loose and stuck the employee on the back of the head/neck.

Working inside the cooler installing bars. Went to transfer a bar to its correct position and lower back popped.

Injured was putting crib blocks under the miner because it was stuck in the intersection of the #10 entry. While putting the cribs under the machine EE smashed two fingers between the crib block and the ground. X-rays revealed EE had fractured both fingers.

EE was conducting a WPE of the XS leach pad when EE stepped out of the truck and onto a rock. This caused the EE to roll their ankle & fall to the ground. After getting back in the truck EE stated that EE was in pain. EE was taken to clinic & diagnosed w/fracture to fibula. EE was taken off work.

Injured employee states was returning to the job site along the belt line. EE states looked up and a piece of loose material fell and struck EE knocking glasses off and hitting EE's eye. Employee is off work since the incident with impaired vision and grade 4 hyphema in left eye. Investigation is still ongoing.

Ee was scaling off a hanger plate. A loose piece of shotcrete fell and hit the ee in the left eye and on the nose. Ee was seen at the ER, diagnosed with a corneal abrasion, blunt eye trauma, and iridoplegia. Ee was referred to an eye doctor and released to restricted duty on ee's next scheduled shift.

Employee was shoveling coal from underneath a low seam conveyor belt when EE felt severe pain in the lower back. On 9/29/2021 employee was scheduled for a MRI, the outcome is to be determined.

The EE was tramming the drill in the pit. The door was latched open. While moving, the door shut onto the EE's left hand. At the time, the injury consisted of minor lacerations. On 9/7/21, the EE's finger was swollen. We took EE to the doctor and EE had a minor fracture thus making the accident reportable.

Employee was working on One Jaw Crusher when the left hand was pinched by the shims. This resulted in two broken fingers and required surgery.

EE lost all strength in the arm for a few seconds. Seen at Golden Valley 9/2, diagnosed with left biceps tendon rupture. Referred to ortho consult. LTI

Miner was placing a piece of tin on a small shed roof. The tin started to slide, miner went to stop the piece from sliding, edge of tin caused a laceration between index and middle finger on left hand. Miner received 2 sutures. Miner was given gloves to wear per company policy but decided at some point not to wear them. Cause of injury was not wearing protective gloves.

EE reported the event the following morning as EE began to feel pain and noticed swelling, but did not request medical treatment until 10-06-2021.

When a site maintenance technician lit a torch, they heard a pop under their arm. The oxygen hose had burst, resulting in fire, causing first and second degree burns to the worker's upper arm and back.

EE was cleaning catwalks and washing the wash plant. when EE was done they started walking down the steps but forgot something and walked back up the steps. EE then went to walk back down but tripped over the hose that they had just used. EE slid down the steps landing on their buttocks bruising lower back

Checking the coolant level on the 982M. To do this the hood is raised on the machine and the employee climbs onto the rear step and visually inspects the coolant tank. EE did so and while climbing down from the step EE slipped, landing on the ground on their feet. EE was removing nose cone and pin assembly from damaged pull yoke on Haul Truck. As EE was attempting to remove the nose cone bearing a piece of the broken bearing fell out of the assembly & struck the back of their hand. The caused a deep laceration. Employee had removed cover guard to put oil in reservoir. EE set the guard beside the reservoir and after adding the oil, EE and another employee went to put the guard back into place. EE initially received no treatment, but late last week was then seen by ortho and had treatment and was sent for therapy.

Pumper began receiving chiropractic care on 10/01/2021.

A mine mechanic was attempting to drive out the front pivot pin from a roof bolter using a 20# sledgehammer and a T-handle drift pin. The employee missed a swing, which resulted in the employee's right index finger getting caught between the handle of the sledgehammer and the drift pin. The finger was lacerated along the top and required 7 stitches.

Employee was working on dozer, when EE noticed a socket under side hatch door. EE attempted to remove socket and when EE did it fell on EE's left hand index finger area.

Employee was helping change the inner boom of the miner CM02. While guiding the hoses that run through the boom, the boom shifted from the front causing the boom to strike the employee's right thumb between the machine stops and the boom itself. EE and a bolting partner were dropping the bolter cable. The cable slipped through EE's hands and slid down arms striking them in the mouth. This resulted in a broken tooth.

Employee stated that they were lifting a full bottle of oxygen to place in the holder on the mantrip when they felt something pop in shoulder. Employee was released and worked regular schedule. On 2/22/2022 employee had rotator cuff surgery to repair a tear and was released back to work.

Employee was connecting the third steel to the second steel. Upon doing so, employee got glove stuck in between the connectors. Employee immediately tried to drop the drill head back down but accidently spun the heads instead, causing hand to wrap up around the drill steels. Employee received a laceration to the left ring finger requiring six stitches.

Flash burns (welding)

Contract EE was assigned to do fire watch for a hot work job. During the part of the morning, the contract EE had watched the welder welding and had looked at the arc flash off and on approx. 15 min before another employee told EE not to look directly into the flash. EE was fine when EE left work and EE's eyes were irritated in the morning when EE woke up.

The employee received 4 sutures due to this incident.

EE grabbed one of the buckets to take it off the loader and when EE stepped off the loader the bucket handle came out of the bucket and slipped through EE's hand hooking EE's glove and puncturing right ring finger causing a laceration on the finger.

While reinstalling the water line manifold on the end of the pipe, the manifold fell approximately 4 ft. from the end of the pipe. The employees left ring finger was caught between the manifold and the belt rail, resulting in a laceration requiring sutures to close.

An employee was driving a OB truck when EE hit a soft spot on the road. The employee reported to EE's supervisor at 9:45 a.m. that EE was experiencing back pain and wanted to seek medical attention. A weathered rope located on an exterior catwalk, exposed to the elements, was being untangled by an employee. The rope began to fray and splinter into small pieces. The wind caught the rope fragments. One of the fragments went into the employee's right eye.

A plant mechanic was changing out bags in the kiln bag house when ppe failed and allowed kiln dust which is slightly alkali, to make contact with a hand. The employee was wearing a Tyvek suit with gloves taped at the wrist but the tape failed allowing the material to break through without them realizing this. This resulted a burn requiring medical treatment.

Workers were finishing up crusher inspections in plant when a worker was stung one to two times in upper right shoulder by hornet. Other workers in the area observed the sting. Worker stated EE was not allergic (that EE knew of) but after 10 minutes EE experienced anaphylaxis.

EE had an allergic reaction which EE never had before in EE's life, causing another EE to call 911. EE was in the ER of about 2 hours then released and then came back to work.

Employee was changing out worn miner bits and was using a hammer and a chisel in this process. When hitting the chisel a sliver of metal came off and imbedded in EE's left forearm. EE continued to work until 10/06/2021 when EE had it removed and received 3 stitches.

EE was working on a conveyor. EE tripped on the tongue and fell along side of the material hopper catching the inner part of EE's thigh on the hopper and received a cut. EE was taken to the hospital, received stitches and returned to work.

A miner received treatment for a foreign body in the left eye, and was issued a prescription.

While replacing a shield on the section feeder, employee pinched right pinky finger between the shield and frame of machine. Resulted in a fracture and six (6) sutures.

Employee strained the back on the right side after stacking a tire on the pallet for storage. The injury did not constitute medical treatment until October 6th when EE was prescribed physical therapy to treat the strain.

Upon standing and moving away from the desk, employee's foot got caught on the computer mouse cord and EE fell, hitting their right cheek on a desk, causing a laceration.

Employee was in Tower 1 moving screen 1. Cut their hand while in the act of moving screens from one side of floor to the other. Small cut.

EE was holding drift pin to knock pin out of upper frame on cone crusher. When the pin came out the upper frame shifted causing EE's finger to get pinched between cone and base plate of cone.

As a utility operator was descending from the bricking rig platform, EE's foot slipped. EE tried to catch self with one hand on the platform access swing gate, and EE's thumb was pinched between the vertical member of the gate and its support post. EE received stitches and was diagnosed with a fracture to the tip of thumb.

Employee was maneuvering becket end of pendant lines into a spool for storage. Employee had fingers between cotter key keeper on the pin and becket end. When the becket end was positioned onto the spool, back of pin made contact with spool sliding pin away pinching finger.

EE attempting to clear a feeder plug on line 6 with a pry bar. EE dislodged small rock causing the larger rock to slough. The crane hook used to assist with the plug released hitting EE left hand.

Employee Was Removing Driver Side Hoist Cylinder from haul truck. The hoist came down on the drift and employee caught right middle finger in pinch point causing a laceration to right middle finger 7 sutures.

The employee was working on green tagged scaffold approximately 8-10 off of the ground around a raw mill roll pair removing the hub off of the shaft. The scaffold boards were against the hub until the housing was removed allowing for a space at which time the employee fell.

Miner climbed a platform to grease feeder bearings. Area is inaccessible. Miner was not wearing a safety harness. Miner finished greasing bearings, miner stepped back and fell through an opening in the handrail of the platform. Miner fell approximately 15 feet onto a pile of rock. County Emergency personnel called to treat Miner.

The employee had picked up a small metal step to take to another location. When the employee turned they felt a dull pop in lower back. The employee did not report until 09/16/2021. They went to a doctor on 9/17/2021 where they were prescribed physical therapy, making the incident recordable.

Lining piece of stone on recut table. When pushing it against guide, cut middle finger on left hand. Was taken to local ER and received 4 sutures. Doctor's excuse out until sutures removal on September 13.

Employee was cleaning out a plugged chute on the rock crushing/screening plant hitting EE's right hand/thumb against the ironwork corner on the plant. This resulted in a laceration of the thumbnail and nail bed. Bottom part of thumbnail was cut off and sewn (2 stitches) back on as a protective cover for the nail bed healing and a preventative for infection.

Employee was attempting to install the rear belly pan on a D10T dozer. Employee had the belly pan supported with a chain that was draped over a cross member to hold the belly pan. As employee was lining bolt holes up with a pry bar one corner came unhooked causing the pry bar to hit employee in the left wrist.

Belt press attendant was cleaning a floc mix bowl, after flushing out the bowl EE went hang the hose up and slipped backwards due to floc on the ground. EE extended EE's hand out to catch fall & got a cut from a metal grading step. After EE fell, we asked EE if EE was okay, EE said ""yes"" we helped EE clean the cut on EE's hand after that EE was taken for more medical attention.

Employee was hitting tooth pin with metal hammer. Piece of metal broke loose from pin and hit lower left leg.

Employee was being transported in a bus to end shift. As the bus was traveling, it encountered a series of bumps that caused the bus to bounce. Employee was propelled off EE's seat impacting the top of the seat in front of EE due to EE not wearing a seat belt.

Employee was preparing to removing a camlock from the rear of the water truck. It was stuck, so they attempted to tap the fitting with a hammer and it had pressure built up and it struck the employee in the mouth.

Employee was cleaning up area around dryer. As they backed up the wheel barrow started twisting causing them to loose balance & roll their left ankle.

While servicing the dredge, EE slipped and hit leg on a 10"" pipe causing an injury below the knee. It was swelling so EE went to get it checked to make sure there was not any damage. Returned right back to work.

Contact with hot objects or substances

The injured employee (IE) was performing maintenance on a section of guarding when they placed left arm on an area that was still hot from using the cutting torch. The IE said left arm brushed up against the hot metal and it was enough for the IE to sustain a minor burn to the forearm area. IE was treated at local urgent care clinic and prescribed bactroban antibiotic.
