An unplanned roof fall occurred in the #4 at spad #4297. This fall is approx 1800' from active section in the return airway. Area was developed 08/31/09. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Size of fall 20'w X 80'l X 5' thick. State and Federal agencies notified immediately.

Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Size of fall 20'w X 100'l X 6' thick. State and Federal agencies notified immediately.

An unplanned roof fall occurred in the #6 entry 2 breaks inby spad #4322. This fall is approx 2800' from active section in the return airway. Area was developed 10/15/09. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Size of fall 20'w X 20'l X 8' thick. State and Federal agencies notified immediately.

Accident type, without injuries

#3 hoist tripped on ""Drive Converter A"" fault and ""Vibration"" fault. Maintenance Coordinator and electrician were called out of the mine to check and reset the hoist. The hoist was back in service at 1:37 AM and then tripped again at 3:15 AM. The hoist was reset again and back in service at 3:40 AM. There were still two escapeways during the incident.

A non injury. High angle slip mainly on the 001 MMU. The #2 shield was caught. There was damage to the hyd system. A retreiver was used to pull shield out.

Rock fall occurred on 1 East Mains in the #5 entry at break #51 and extending to break #52. Fall measured approximately 20' wide x 50' long x 4' high.

Our examiners found a roof fall in the 5 Main North worked out area in entry number 3 just inby the intersection of crosscut 41 and it extended thru the intersection of crosscut 42. The fall was approximately 80 feet long, 18 feet wide and 20 feet high.

Belts had stop running started to investigate why belts were off and found Rock Fall on #1 belt 75 feet underground from surface. Fall was 22 feet long 18 feet wide and 15 feet hight.

An unplanned roof fall occurred in the entry #1. 1 break inby spad #4319. This fall is approx 2800' from active section in the return airway. Area was developed 10/15/09. Roof fall did not affect ventilation and/or passage of persons. The fall will not be rehabilitated. Size of fall 20'w X 60'l X 8' thick. State and Federal agencies notified immediately.

Accident type, without injuries

While pillaring in #1 entry, 15th row at the left wall, a roof fall has occurred on the Continous Joy Miner. The size of the fall was approx. 19'wide x 8'thick, x 35'long. No ventilation has been effected or miners hurt. Rope bolts, beams, and jacks have been set and additional roof support of 5'and 10' rope bolts will be in installed.

A fall was found by an MSHA inspector on 11/30/09 at 79 + 10 entry 1 in 8 LMS. Fall was in a four way intersection that had been mined 10/29/09 and supported with 4' and 8' full resin bolts. There are no plans to clean up this fall. No injuries resulted. It was reported due to the fact that an intake stopping was damaged.

Rock fall occurred on 1 East Mains in the #6 entry between break #12 and break #13. Fall measured approximately 20' wide x 20' long x 3.5-4' thick.

A roof fall occurred in the #7 entr7y in the old 9A track area in the intersection at spad #23208. The fall was about 30'long, 20'wide, and 6 to 8' high. The area was bolted with 5' torque tension bolts. The fall was danger off, but it will be clean up and rebolted.

A roof fall was discovered on #2 Unit (entry #7) cross cut #22, in the 2nd S.E. Panel. The fall was approximately 90' long x 18' wide x 15' thick. The area was bolted with 8', 10', and 15' roof bolts. The fall will not be cleaned-up.

Hoist #3 was taken down to perform preventive maintenance. While trying to bring hoist back online, some programming issues were found and repaired. No underground employees were trapped; two means of escape were available at all times.

Hoist tripped on ""Drive Converter A"" and ""East Motor Overtemp"" faults. Electricians and maintenance foreman were called out of the mine to check and reset the hoist. The hoist was back in service at 5:45 AM. There were still two escapeways throughout the incident.

EE was cutting holes in tail piece for spill skirt installation. The CO monitor was sounding off. EE could not make the outside man understand how to cut it off. EE left the area to turn off the CO monitor. He was away from the area for a long while so that he could eat lunch. When he returned, he discovered smoke and fire at the tail piece.

A roof fall was found at 11:45 am by MSHA inspector and carried out and reported to safety dept. at 1:45pm. The fall was reported to MSHA hotline at 1:50 pm. The fall was located in the East Main at Break 23-24, in#4 entry, the fall was approx. 18'wide,20'Long,10'thick.The area was timber and dangered off.

During an examination of the intake escape ways by a company and federal ee a roof fall was found, in the pillar line at x-cut 39-40 on the M3 zone. It was supported with 6' fully grouted roof bolts. The fall was 20' long, 17' wide and 8' high.

While hoisting a continuous miner out of slope, the low boy jumped track and the emergency brake set. The hoist was down from 11:20 Am to 3:00 pm.

Bore miner operator went to turn up the face fan and upon returning noticed a floor bleeder that had been ignited under the FCT. The FCT operator grabbed a fire extinguisher and extinguished the floor bleeder. The ignition was not witnessed by anyone but was discovered by the bore miner operator.

On 11-23-09 the main hoist suffered a control module failure at the #29 mine. The hoist failed at 2:34pm and MSHA was notified at 2:37pm. The hoist was back in service at 5:00pm and MSHA was notified at 5:05pm.

During the normal weekly Airway walk, an examiner discovered a roof fall blocking the aircourse. There was no damage to equipment, and no persons were injured as a result of the event. This occurred at the #9 X-Cut line in the old 23 Mains area.

A roof fall was discovered in the #3 entry of 2-2 butt room in abandoned section. The fall was 80' in length and 20' in width. No equipment was involved. The fall was discovered on the weekly air course run.

Face of #4 Entry in the South Main Section at 1950 outby to 18+57. There was 8' cable bolts in the inter. 19+23. The fall was about 8' high just above ankorage zone. The fall measured approx. 100' long. According the the report from prep-foreman, the roof started working in the inter. 19+23 and the face of #4. They went for cribs & approx 15 minutes later it was on the bottom.

A roof fall occured in the #5 entry intersection 70' inby survey station # 3966 of the 003/004 mmu. The fall was approximately 40' long x 20' wide x 7' thick. MSHA was notified at 12:40 pm on 11/20/2009.

Roof Fall in #7 entry approx. 2000' outby section. Fall was approx. 60'long X 18' wide X 6' high.Top was heavily laminated in arch of fall. Area was cribbed and dangered off from all directions. Stoppings were constructed around fall area.

A fire started in the engine compartment of a scaler, mine was evacuated and MSHA. notified within 15 minutes. After 18 hours mine rescue teams put the fire out.

Two breaks inby spad 4771, a roof fall was found. EE & mine inspector found the fall. The size of the fall was 20 ft by 20 ft long and 6 to 8 feet high. The fall will not be cleaned. The fall is timbered and dangered off.

A fall of the roof occurred in the S.E. Life of Mine Mains C at x-cut 433 in the belt entry. The fall measured 60' in length by 20' wide and it was 5' to 6' thick. There was no personell, equipment nor ventilation affected.

A roof fall was discovered in Entry #4 (Cook Portal), the fall measured approxmately 80ft long by 20ft wide by 10ft high. No injuries resulted from this fall.

#29 mine hoist lost power at 8:53am due to electrical failure. MSHA was notified by phone at 8:58 and MSHA was notified again at 11:20 that hoist was back in service.

Employee entered bulk storage tank at Retsof Bagging Facility to dislodge a blockage of salt in the bin bottom. Employee hooked up to fall protection but not a life line. When salt dislodged, employee was engulfed with salt up to his arm pits. Employee was removed from the salt safely. Employee was not injured.

A fall of roof occurred in the South West Sub Main 6-7 entry, # 30 corsscut. The roof fall was approximately 20 feet in length x 10 feet in width x 7 feet thick. The area was supported with 6 foot fully grouted roof bolts.

A rock fall was found in the East Mains #5 entry brk. 22 intersection off the section, no ventilation, or machinery involved, no miners hurt. The fall was approx. 5' thick, 20'Long, 15' wide. The affected area has been dangered off.

Roof fall at No. 11 & 12 x-cut, No. 1 entry of 1st left panel off of 1 west mains. fall was 20'W x 70'L x 3'-6' Thick. Cribbed and dangered off area, will not be cleaned up. Area bolted with 5' torque tension 3/4"" roof bolts. Immediate roof was shale with laminated sandstone above it. Fall inby loading point.

A roof fall occurred in the 1st West Submain on #1 unit inby spad number 7+00 in the face of #7 entry measuring approximately 50ft. long x 19ft. wide x 8.5ft. high. This area was supported with 5ft. fully grouted roof bolts and 9ft. cable bolts.

A roof fall was found by the asst mine manager at 3:18am and reported to MSHA at 3:30am. Fall is located at X-cut 43, Entry #3 of the M-3 zone. It is 20' side, 25'long, and fell 12' high. The area was supported with 6' fully grouted roof bolts and was developed on 8/15/09. This fall did not affect ventilation, passage of miners or cause personal injury to any miner.

While mining #4 pillar at brk. 24 a roof fall occured on the continous Joy miner, the size of the fall was approx. 11.5'W x 10H x 8' thick. 48"" resin bolts and 7' timbers set ,tests holes & straps according to plan. No ventilation affected or miners hurt.

A roof fall exists at the 21 room intersection on the north east mains belt line. The fall is approximately 35 feet long and above the bolt anchorage zone. All agencies were notified according to law.

An unplanned roof fall was discovered at approx. 7:10 p.m. at 69 break on the 2 south belt entry. The fall measured approx. 4'high x 20' wide x 50' long.

An unplanned roof fall occurred below anchorage but had roadway blocked in primary escapeway. Men were withdrawn & escapeway rerouted. This was phoned in to hotline. The area was located in 1 crosscut inby SS 1536 adjacent #4 belt. The area has been rehabilitated. A cleanup rehabilitation plan was submitted.

Employee was a passenger in the mantrip when it ran into a rib while sliding thru a curve. Passenger hit his head on the front plate of mantrip. Per procedure employee was life flighted to hospital. No injury was reported.

A roof fall occured on 4A belt xc-32 spad # 21+79. The size of the fall was 40ft long, 20ft wide and 6-8 ft high.

A roof fall occurred at the #1 section, 1 x-cut outby spad #46221 in the intersection. The fall was about 6'- 8' high, 20' wide, and 30' long. The area was bolted with 5' torque tension bolts. The area was timbered and dangered off.

A unplanned roof fall occurred in the #2 Gas Mine in the #6 entry at spad #348 that measured 20' wide X 17' high. No injuries were reported.

Vibration cause a wire to break on second stage contactor resulting in loss of second gear. the mine was evacuated at 3:52 am and complete at 4:05 a.m. The hoist was repaired at 4:15 a.m. and the hoist was tested. Miners returned u/g at 4:20 a.m.

The elevator serving #6 portal was inoperable due to the doors being off track from 2:35 p.m. until 3:05 p.m.

This report is being submitted under protest. Co. does not believe this incident meets the criteria outlined in 30 CFR Part50 defining an accident. At approx 2am, a greater than 1.0% of CH4 was encountered at the last strap at the top of the wedge in the 8A face. This CH4 was encountered during the investigation of an uncharted void which was intersected in left rib of the seam.

An unintentional roof fall has occurred at the #7 entry at the number 20xc of the mmu-001 section. The fall was bolted with truss bolts and cable bolts and five foot point anchors. The fall was approximately 17'10"" wide and 8.5' feet high by 20 foot in length it contained in the intersection. All approaches to the fall were timbered off.

An unintentional roof fall was discovered on 2 SW Mains in #6 entry(intake) S.S.1201, approx. 2500' off the working section. The fall measured 40'L x 8'H x 18'W. The cause of the fall appeared to be a slip with a coal rider seam and shale roof. This fall does not interfere with ventilation or with escape travel and will not be cleaned up at this time.

During weekly exams, examiner found a roof fall at BK #20 of North Mains in #3 entry (old roadway). Area what fell is 20' x 20' x 8' in intersection where spad # 6811 was located. Area was timbered off and dangered off. This is an old area, non-producing. This does not affect ventilation to working section.

The intersection of No. 1 Entry, at XC-11 in LW-304 had been dangered off due to poor roof conditions. Crews were progressively installing roof to floor supports toward the intersection. As pumpable cribs were pumped, they observed the roof sag. The roof eventually fell, 18ft W by 60 ft L by approx 15 ft high.

A continuation of an old fall was discovered by the mine examiner at spad #6582, crosscut #89 in the #7 entry. Fall measured approx. 18' wide x 10' long x 7'-8' high. No injuries, equipment or ventilation were effected. A partial clean up will be done. The area was timbered off.

A roof fall was discovered in the Main West 1, at crosscut 41, in # 9 entry and extending into the crosscuts. The fall measures approximately 110' long, 17' wide, and 9' thick. The fall is not blocking any route of travel or escapeways.

A roof fall was found in the East Mains at brk. 22 in the #1 entry, the fall was approx. 15'W, 20'L, 6' thick and Brk. 23 in #2 entry, the fall was approx. 15'W, 50'L, 6' thick. Ventilation was not affected, no equipment or persons involved. The area has been timbered and dangered off.

A roof fall was discovered in the #4 entry of the 2 mains butt, 1 x-cut inby spad #11274. The fall was 60' long, 20'wide and 42 "" high. No equipment was involved. The area was cribbed, posted and dangered off. This section was abandoned and equipment pulled out before the fall.

A roof fall was discovered by airway examiner in worked out area. Fall was timbered and dangered off. There was no equipment involved and no interruption in ventilation.

A roof fall occurred in the no. 6 entry between no. 14 crosscut and no. 15 crosscut. The fall measured approximately 20' wide, 25' long and 6' high. There were no injuries or damage to equipment from this occurrence.

An unintentional roof fall occurred in the #2 entry of 1 left pillar section. The fall was approx 80' X 20' X 8' and started with a normal pillar fall and came outby over the riding of the Breaker timbers. No one was working in the area at the mine.

While extracting coal in the #2 face of the 9E section, 75 feet inby crosscut #28, the continuous miner operator thought he saw a flash on the right side of the ripper head after the methane monitor had spiked and de-energized the machine. After investigating, no evidence of an ignition was found.

After taking a cut right hand break out of the #7 entry the miner moved to another cut and the bolter operater seen the place was working and backed the bolter out and called for the boss and the place started working more and then fell starting from the inter into the right brk. they had just started the fall was approx. 8 ft wide, 4-5 ft thick and about 25 ft long.

Rock fall occurred on 3 East Mains at break #25 between the #1 and #2 entry in the crosscut and adjacent to the #8 conveyor belt. Fall measured approximately 18' wide x 40' long x 6' thick.

An unintentional roof fall occurred on the belt line two breaks inby spad #4361. The fall measured 50' long 20' wide and 6' to 7' thick. The fall did not affect the air flow or the travel of men.

Spoil side slide in B-Pit. The spoil slid in at the south end of B-pit. It covered the ramp that was used to enter and exit the pit and blocked four employees in that were working 2000 ft. away from the slide. No injuries occurred with this slide. The shift started on 11/6/2009 at 7:00 PM. The slide occurred at 2:15 AM on 11/7/2009.

An unplanned roof fall occured on the 1st Southwest sub main off the 2nd Southeast sub main in entry #6 at cross cut #8. The fall was on the supply road and will not be cleaned up. The area was roof bolted with 6' and 10' roof bolts. The fall appeared to be fractured stack rock. The fall was approximately 17' W x 17' L x 15' H.

Roof Fall in 4 East occurred above the bolt ankorage in the Intersection of #4 Entry at 4+ 18 in the Last Open Crosscut. The fall was approximately 80' in length and 8' high. This happened between shifts and was found at 12:10 AM by the 3rd shift prep foreman.

An eletrical glitch in the system caused the elevator to stop asending. Two persons were on the elevator at the time and were confined to it for approximately 2 hours. The elevator service company found the glitch and reset the system. The elevator was tested and found to be operating properly by the elevator company.

An ignition occured at 103+79 on the E-20 longwall face at #10 shield. The ignition was self extinguished by the shearer water sprays. There was no damaged to equipment and there were no injuries.

This was an extension of a roof fall that was discovered 10/15/08 on 2 South, #3 entry, #4 x-cut. The secondary escapeway was blocked, but it was re-routed to the #3 entry. The fall was approximately 40' x 30' x 10' thick. Area was mined August 2006, bolted with 6' fully grouted roof bolts. Initial fall had been timbered off.

An unplanned roof fall occurred in the #2 gas mine approx. 70' inby spad #1040. The fall measured approx. 50' long x 18' wide x 12' high. no injuries were reported. The area was developed 9-10-08.

An unplanned roof fall occurred in the #2 gas mine at spad #1262. The fall measured approximately 19' long x 20' wide x 12' high. No injuries were reported. The area was developed 1/6/09.

Discovered a roof fall in the GL4 panel 1 break left of spad #2594. Fall is 7' thick x 19' wide x 40' long and 7' thick x 19' wide x 30' long through the crosscut. This area was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 7200' from the section and has been dangered off.

Discovered a roof fall in the GL4 panel 1 break left of spad #2580. Fall is 4' thick x 19' wide x 75' long, was bolted with #5 x 48"" fully-grouted resin roof bolts. This area is 7200' from the section and has been dangered off.

Preheater Cone Valve stuck open and the Raw Mill Cone Valve stuck closed causing the Preheater to go positive. The Preheater Fans shut down and hot kiln feed fell out of the Preheat Tower and into the Kiln and Clinker Cooler, then onto the Pan Conveyor mixing with the clinker. This hot materal conveyed to the rubber conveyor belts which caught fire and were destroyed.

A roof fall was discovered on the N0/2 belt conveyor at cross cut N0/11. The roof fall is approximately 25' long X 25' wide X 7 1/2' high. The roof fall is on the belt conveyor. The roof fall area will be cleaned up and re-supported. The belt conveyor will be re-installed.

A fall of roof has occurred in the South East Mains between crosscut 53 and 54 in entry # 2. The roof fall was approximately 30 feet in length x 18 feet in width and the thickness is unknown. This area was supported with 6 foot fully grouted roof bolts.

A roof fall was discovered in the number 3 entry, number 2 belt at crosscut 40. The fall measured approximately 18' wide by 30' long by 8' high. No injuries resulted from this fall.

There was a fall on the #4 Mains Beltline, #42 Crosscut, #6 Heading. The roof was bolted with 6' fully grouted roof bolts and 12' PT Cable Bolts. The was fall 3,200' outby the closest face. No persons, equipment, or ventilation was affected.

Weekly examiner discovered a roof fall in the 8 Haulage bleeders between 8K and 8L. The fall measured approximately 20'L x 20'W x 12'H. The area has been dangered off and a different route will be used.

A fall of roof has occurred in the South East Mains at crosscut 54 in entry # 3. The roof fall was approximately 30 feet in length x 18 feet in width and the thickness is unknown. This area was supported with 6 foot fully grouted roof bolts.

An unintentional roof fall was discovered on 11/3/2009 by mine examiners conducting their weekly exam of an abandoned area of the mine. The fall was located in XC #5, between entries 4 and 5 of the 1 Left/5 main North. The fall measured 18' L X 18' W X 8' Thick and was bolted with 6' resin bolts. The area was developed in 5/07.

An unplanned roof fall occurred in the #2 entry @spad #3569, approx.4,000'from active face.Roof fall measured 80' long X 20' wide X 7' thick. Fall occurred in the return airway. Airway was not affected and will not be rehabilitated. State and Federal agencies notified immediately. This area was developed 9/15/09, area had been supported by 60"" Torque Tension bolts.

An unintentional roof fall occurred on the 1 left pillar section at the pillar line in the the #3 entry. Two Fletcher MRS's were trapped by the fall. The area was cribbed and timbered off. The immediate area was resupported and the MRS's were retrieved.

The elevator was traveling down the shaft with four employees. As the elevator approched the bottom of the shaft, the elevator traveled pasted the landing by a few inches. This allowed the elevator doors not to open, the elevator was re-set and the elevator brought back to the surface, where the employees unloaded the elevator at approximately 2:08 pm.

A roof fall was found at 1 South Mains 40 feet outby SS# 923, in the #5 entry. Size of the fall is 70' wide X 60' long X height unknown. Ventilation and travel is not affected and the fall will not be cleaned up.

At this point in the investigation, it appears that the boom or boom hoist ropes came in contact with an adjacent structure as the boom was being lowered. The elevation of the boom changed suddenly as the boom hoist rope or boom lost contact with the structure, striking both individuals.

Employee was traveling #2 Longwall Bleeders and when he did not check in, we sent someone in to look for him. He was found unresponsive with no vital signs. The cause of death is still being investigated.

EE was tensioning 10' post tension cable bolts in the #2 S. belt track entry. While holding the tensioner he was unaware that his left index finger was between the ram and the tensioner. When he instructed his co-worker to retract the ram the tensioner retracted amputating the tip of the EE's left index finger.

Employee finished shift and was off for the next two days (incident happened on Monday in the AM, employee returned to work on Thursday at 8:00 a.m. EE's wife brought in doctor's note on Thursday at 11:30 a.m.

The loader operator was standing on the back deck of the front end loader cab cleaning the window. The deck was free from any hazards when he stepped backwards away from the window his foot became entangled in the safety railing causing him to fall first upon the tire and then to the ground breaking his arm.

EE stated that he had a shoulder sprain.

EE was exiting a Volvo A30 to empty a bin when he slipped & fell on truck steps during rainy weather. He hit his elbow against the side of the truck causing bruising and pain to his elbow and shoulder. No other damage to him or the equipment was noted. He informed his supervisor of his injury who arranged for IE to be taken to hospital for treatment.

Metal flakes from grinding got on the EEs clothes. Later the EE was dumping a load in the hopper with a haul truck with the window down and the wind blew the metal flakes into his left eye. Went to doctor after work, when it began to bother him, and the doctor remove the metal flakes.

EE tried to roll out of the way but both legs were run over below the knee by the car.

Employee did not have an accident, but came in the following morning and said his leg began hurting at night. He is alleging that his left leg was strained from doing his regular job tasks.

Was holding up inspection door to surge hopper with right hand. When done inspecting, his hand gave out causing door to slam down onto left hand. Injury refused to have his hand checked out immediately but since it would not stop hurting he decided to have it looked at today, 11/30/09. The doctor said it was broken and he is now in a cast and on restricted duty.

Over-exertion in lifting objects

EE was moving a large electric motor up a flight of stairs to a shaker at the main crusher and strained his lower back. The strain was enough for a doctor visit. The doctor prescribed muscle relaxants and 3 days of restrictive duty of no more than 10 pounds of lifting.

Struck by falling object

Employee just started bolting a breast down round. He was drilling when a rock fell off the hanging wall, striking him on top of the left foot. He reported to the medics and was sent down for x-rays, which confirmed a fracture on his left foot.

Over-exertion in lifting objects

A mechanic was moving an equipment part and as he lifted it he strained his groin resulting in a hernia.

Over-exertion in pulling or pushing objects

Checking density on truck and saw a cap falling in hole. Bent over to get it and felt a pain in back.

Employee was removing the 1"" spline drive impact tool from the tool box located at #1 rod mill. He felt a pull in his lower back. EE placed on restricted duty.

Struck by falling object

Maintenance tech had a piece of metal 1/2"" X 6"" X 72"" clamped to the side of the low boy trailer. The metal was hanging over about 4"" and was clamped at each end. He was grinding a 45 degree angle to the metal with a large grinder, during the grinding process with some pressure applied, the metal slipped and fell with one end landing on tech's right foot.

Fall onto or against objects

Employee was helping remove bolts from a pump. With two bolts remaining, employee decided to go under the suction line to the east side of the pipe due to restricted working room and possible line of fire issued should the pipe move. When he crouched down to step under the pipe, his foot slipped on the metal.

Mobile Equipment Operator was dismounting Rubber Tire Dozer at end of shift, while stepping from the last step, felt a pop in the left knee. Employee didn't initially require medical treatment. Employee visited ER after shift was over and was placed on modified work after scheduled days off beginning on 11/21/09.

Subject changing a belt roller. He stepped back and rolled his ankle.

Acceptable conditions- Employee was removing equipment from belt structure and got his finger caught between two pieces of equipment causing a fracture to the left pinky finger.

The employee went to the company clinic on 11/14/09 and was placed on restricted duty. The employee was using a putty knife to remove a decal, when the knife tore through the decal and struck L-palm, causing a laceration that required sutures.

Struck by flying object

The hopper door was opened by an employee. The injuried employee had a piece of plywood he was going to prop the door open. Employee was going to go up the ladder when he looked up and dust fell. The dust landed on his right eyebrow and behind his safety glasses. Employee went to washroom and flushed his eye. Employee went to Doctor, was treated and put on light duty for five days.

Employee was adjusting the leveling rods on the left drill pot of the roof bolting machine. He was using channel locks to turn the adjustment mechanism on the rod and had his left hand resting on the drill boom. The drill boom dropped and his left little finger was pinched between the rod and boom frame.

Bodily reaction, (Not Elsewhere Classified)

EE was putting drive motor on the cooler in a restricted area. He twisted awkwardly injuring his knee. He was seen on 12/22/09 and continued working while the treating physician wanted to await progress. A follow up 2010 MRI revealed a torn meniscus which was surgically corrected on March 2, 2010. Since then, the ee has been placed on restrictions awaiting complete recovery.

EE was in a basket about to install a new airline when some ground came down on him. He had hand scaled the area prior to beginning work. EE received stitches to the elbow and bruised ribs.

While installing side plate on Fine Ore Feeder, the side plate caught the EE's right wrist between the side plate and steel beam.

Over-exertion in lifting objects

While at work an employee began to experience pain in his left shoulder when pulling up the boarding ladder on P&H 2300 shovel. The employee saw an orthopedic specialist and had surgery on 11/12/2009 for a left torn rotator cuff. The employee was placed on restricted duty.

EE was trying to unplug the chute on the jaw crusher when a plate fell down in the chute and he was using a prybar to try to pry the plate loose when his finger got caught between the bar and the chute. This pinched his middle finger on his left hand. His finger is fractured.

Fall to the walkway or working surface

Employee was in an upright position operating a Fletcher DDO-15 Roof Ranger Roof Bolter, when he slipped on a piece of rock on the mine floor. In an attempt to break his fall, he extended his right arm out and when he landed, he received a strain to the right wrist.
