Accident type, without injuries

Continuous miner cut into a degas hole that was in the top instead of in the coal seam. The sparks from the miner bits cutting the rock caused an ignition of a pocket of gas in the degas hole.

An unplanned roof fall in an inactive area has occurred in the Wyoming No. 2 Mine inby spad 3673 extending to spad 3781 between the 2 blocks. The fall is approximately 70 feet long by 19 feet wide and 7 1/2 in height. This area of the mine was developed approximately 11/201/2013. No injuries, no equipment involved. No ventilation controls were effected.

On 3/31/2015 at approximately 10:50 a.m. a roof fall was discovered that was not located in active workings of the mine. The fall was located in the previously mined 6 West Panel in the #4 entry, in the intersection 1 break inby spad 3129. The area was dangered off and cribbed.

Driver fell asleep drove over adequate berm on wrong side of haul road over embankment hit trees. Taken to emergency room, he was drug tested checked and released. On way back to job told owner he had sleep disorder. Not back @ work because truck frame is bent and hood busted. MSHA inspector not released truck yet and we are concerned about sleep disorder we did not know about.

An unexplained roof fall occurred at break 86 and spad #536. The fall measures 19' wide x 20' long, and 10' high.

A malfunction of the hoisting car occurred at 5:22pm caused by two fuses blown in the control panel.All sections and personnel were notified at 5:25pm, production was halted and employees withdrawn to outby the loading point.The hoisting car was repaired at 6:39pm and crews were notified they could return to work.No employees were injured or killed as a result of this incident.

On 3/30/2015 at approximately 11:00 p.m. a roof fall was discovered on the 1 South Mains at break 31 in the #7 entry. The fall was dangered off and all approaches breakered.

Upon examination it was found that the 20 man elevator at the Main Streets portal was inoperable. Maintenance was notified to make necessary repairs consisting of replacing the lower limit switch and adjusting the linkage on the governor rope. Elevator was back in service at 6:35 AM

While navigating a left hand corner on the Rain access road a Caterpillar 735 articulating water truck over turned and caught on fire. / Damage to the tractor portion of this equipment.

The area where the roof fall occupied was in a transition zone where the shale roof was turning to sandstone. Supported but it fell above the supplemental support. This fall was on a belt line that had been developed 3 years prior to the fall. The only damage was to the beltline for approx. 30 feet.

A miner operating a Caterpillar D11N Bulldozer in the 002-0 Packer 5 pit was uninjured when a portion of the south ""bottom rock"" highwall unexpectedly failed. The bulldozer was engulfed in the collapse and partially covered by rock and rubble. The bulldozer sustained serious damage and remains in the debris field. The miner exited the bulldozer immediately without assistance.

At 2:15 pm on 3.28.15 the electrical power wire fell down the shaft. The electricians and utility personnel were called to make needed repairs. The shaft was inspected and cleared for use at 1:55 pm on 3.30.15.

A roof fall was discovered in the bleeder system on the 5th West Mains between SS#9791 and SS#9758 approx. 2 breaks in length and to a height of 5'.

AMS operator saw on the monitor that CO levels were over the spontaneous combustion plan at the mine.

An malfunction of the hoist car occurred at 3:24pm caused by the brakes being unable to be released. All personnel were notified at 3:25pm, production was halted and employees withdrawn outby the loading point. The hoist car was quickly repaired at 3:35pm and crews were notified at 3:36pm to return to work. No persons were injured or killed as a result of this incident.

At 10:50 am the fish creek yard experienced a power outage which cause the elevator to stop 12 feet from the top with ee trapped. He was entraped for 45 minutes when power was restored.

The elevators at 14 mile was down due to the software being bad. Brought all the men outside while the elevators was repaired it was ready to go at 7:00PM.

Hoist man was pulling muck off 52-pocket; #1 side reached 4300 and #2 side was at 1200. Hoist shut down - tried to reset-all attempts failed. Computer was locked up. Programmer in Canada reset on-line. Programmer investigating fault.

The slope hoist was down from 11:00 am till 5:30 pm on 3/24/2015. The auxilary brake on the hoist gear box was sticking. This condition was repaired and the hoist put back in service.

Isolated roof fall in remote South Portal entry No. 1. In this area there was a thicker lens of mudstone and a steep slope on the surface. Cover was 300' and the area was bolted with 5' bolts. Entry width of 20ft. No personnel injured, no interruption to ventilation. Roof fall was re-bolted and supplemental support was added in the affected area.

A non-injury roof fall accident occurred at this mine at approximately 09:30 a.m. The roof fall occurred on the active 007 & 008 MMU's in the number 9 entry across from survey spad #8862 two breaks outby the section dumping point and a second fall occurred in the bleeder inby blocking both bleeder entries. A 103 (k) order was issued #8295131.

An unplanned roof fall occurred at approximately 3:15pm. The fall was at R-seals between #2 & 3 cross cuts in the 1-2A panel. the fall was about 9 foot high, 20 foot wide and 40 foot long. the fall was timber and danger off and will be clean up by a approve plan. The area was mine in 2008.

Found an unintentional roof fall at break #30 on the #22 belt-line. Fall dimensions are approx 30'Long x 20' Wide x 10' Thick.

A roof fall occurred in the number 4 portal (fan) intake air course entry approximately 18 feet wide, 10 feet high and 60 to 70 feet in length. The cause of the fall was due to the extremely harsh winter weather of late where the roof froze and thawed several. This fall was immediately report to MSHA via the 800 number.

A rock fall occurred above the anchorage zone on in the 2nd east belt entry between crosscut 15 & 16. The length of the fall was approximately 65 feet long, rib to rib and approximately 6 feet high. The area has now been cleaned up and the roof supported.

MSHA has determined that an acc. occurred at the Cumberland ROM #2 raw coal stockpile a dozer became stuck on the pile. Operator verifying location of #2 feeder when back of dozer slid into void above feeder. Operator in dozer was never in a position not to exit. Coal was never up to dozer doors. Coal was removed, dozer driven out. MSHA notified.

Galena hoist man called the hoist would not reset. He pushed the E-stop button, left the hoist cab to use the restroom. When he got back he hit the reset and nothing happened. The electricians arrived and check breakers and fuses. Found 2 fuses on the control power box blown. No reason why they blown but replaced fuses and hoist fired back up.

Fire was observed in the Reclaim Belt Tunnel in the vicinity of No. 3 Feeder, which is located under the Stacking Tube. The coal and belt fire was extinguished with water, the concrete stacking tube and surrounding area was inspected and no structural damage was identified. The fire was deemed extinguished by approximately 1:30 pm.

While the lower skip work deck with 3 personnel on the conveyance was exiting the dump pocket the dump arm on the skip became jammed. An ""all-stop"" was given, the condition evaluated and it was determined that the dump arm would need to be cut to free the skip. The task was completed in approximately 1.5 hours.

A roof fall was found on 3/14/2015 at entry 7 at survey station 35+70 in 8L intake, MMU 010. Area had been mined 2/20/2015 and was supported with 8' full resin pins, 4 additional 10' cable bolts had been installed in intersection. Two significant slips likely contributed to failure. No injury or property damage resulted, no plan to clean up fall.

The elevator doors would not open when the elevator reached the bottom landing. The elevator doors were adjusted and the elevator was back in service at 8:05 am.

A roof fall was found in our Primary Escapeway in the 5WD Sub Mains. MSHA was notified of the fall at 9:30am. The fall did not cause personal injury, impede passage of miners or affect ventilation.

Mountain View Mine incurred elevated water levels and inundation in our bleeder ventilation entries due to snow thaw and large amounts of rainfall, which disrupted the mine ventilation system.

The hoist car was removed from service due to a malfunction of its magnet. The hoist car was repaired and placed back into operation.

The Koepe was not available due to a motor needing replaced on the hoist. All employees underground evacuated via the production hoist. There were no injuries or other issues. The hoist was back in service by 1:00 AM.

An alleged dust ignition appeared to have occurred around the cutter drum of the continuous miner at the face. No injury to the miners in the area nor damage to the mining equipment. The incident occurred while cutting in a belt trench with pyretic intrusions. Rules, regulations and policies were complied with.

While performing the daily hoist inspection, the power tripped due to a fault in the process logic system. The hoist was down approximately 83 minutes while the problem was diagnosed and the faulty positioning switch was replaced.

Lost CEI power to the mine. One of the three transformers needed to be replaced. This shut down power to the hoist. All employees evacuated via the Koepe Hoist with no injuries or issues. Power was restored on midnight shift.

A roof fall was found 8 breaks inby surface in intake entry No.7. Survey Station 13831. This fall does not impede travel or ventilation.

Hoist went down at 8:25 am on Monday, March 9, 2015. Electricians were called. After repairs were made to a relay and inspection was made, the hoist was released back into service at 10:15 am.

Haul truck operator backed through the berm on a 30 foot dump. The operator was told to stay in the unit while the truck was secured. Bucket truck was used to retrieve the operator. Operator was restricted in the cab longer than 30 minutes.

A no injury roof fall occurred at the no. 1 mine portal. The roof fall damaged the mine canopy at the entrance to the no. 1 entry. The roof fall was rock from the high wall and the edge of the mine roof for approximately 4'-6' feet underground. Upon discovery of the roof fall, MSHA was notified. The area was cleaned up and a new canopy was installed in the no. 1 portal.

Due to power outage site wide the hoist man was going through the safety check found could not get the skips to move.

Power from the NYSEG owned substation was interrupted due to two fuses blown. This caused the #1 and #3 hoists to stop while performing test trips. There were no people underground at this time.

A fuel line at the kiln burner failed catastrophically spilling MDF on the ground. This fuel then caught fire due to the radiant heat in the area, and was not able to be extinguished with in 30 minutes. No injuries or illnesses to report.

While extracting partial pillar recovery #6 entry the 2nd right cut out of the pillar an unintentional roof fall trapping our continuous miner under approximately 4' to 5' of roof. This area appeared to have laminated roof condition.

Grease was on the bottom pulley of the governor pulley rope causing the rope to jump off the pulley. Entrapping 1 man for 2 hours and 30 minutes

Took plant down on Sunday March 1st for outage. On March 7th, 2015 saw glowing coal embers inside coal dust collector hopper/screw area. Called fire department and reported to MSHA.

Hoist went down at 9:30 pm on Saturday 3.7.15. The skip on the underwind rope did not close because of a boulder and caused a slack rope condition. Repairs were made and hoist was inspected and released at 11:15 pm.

IGNITION - Occurred while cutting coal and bottom rock when a methane frictional ignition occurred on the curtain line side of No. 2 entry on the No. 6 section E10 tailgate. The depth of the cut was approx. 23 feet. The flame burned approx. 10 to 20 seconds and was extinguished with a fire extinguisher.

Control board for the automatic doors failed causing the doors to function improperly while on the landing at the surface. There were no persons injured or entrapped due to this occurrence.

During the replacement of the lanyard switch at the service landing, it became impossible to reset the service hoist. The remote e-stop relay in the safety circuit would not close, thus not allowing reset.

At 9:33 pm a supervisor noticed hot spot on the coke pile, he investigated farther and discovered that infect it was smoldering. he ordered the loader operator to start to open the pile up. at 11:40 am the smolder was smothered completely out.

Unplanned fire on a Kawasaki 90z4-2 Wheel Loader. Presumed to be started by electrical issues. No employee or individual was harmed during this situation.

The resolver motor on the Koepe failed and needed to be replaced. All employees underground at the time of the incident evacuated via the production shaft. There were no injuries or other issues.

An unplanned sulfur fire occurred on the 4500 level of the C4 south wall at 8:45 pm. No equipment, no structures and no miners were put in harm.

The Cameron elevator went down due to the door switch needed adjusted 20 men became entrapped for 45 minutes. Door switch on the car while it was on the bottom of the shaft needed realigned not making good contact.

At about 11:18 a.m. the A skip in the production shaft was found to be out of its guide. Investigators reported a broken guide rail near the bottom landing at 11:40 a.m. Repairs were initiated and the hoist was functional by 6:40 a.m. on 3/3. Damage is believed to be a result of wear. No injuries or entrapment.

Employee was inspecting elevator when a piece of ice fell off the shaft wall and struck the bottom overtravel switch, resulting in an entrapment until 4:30 am. No problems with training, PPE, mining methods or compliance issues.

Elevator shut down (Primary Escapeway) and was blowing fuses and was down for most of the day. It was repaired and restored back to service on the same day.

An unintentional fall of the roof occurred on the B North panel at cross cut #40 in the return air course. The fall located at spad #169 was 80 ft long x 18 ft wide and 18 ft high. A plan to clean up this fall has been submitted to MSHA.

Struck by falling object

The victim was repositioning the block saw near the edge of bench with the saw operating in reverse when he stumbled and fell 4.5 feet to the lower bench. The saw continued in reverse and fell on top of him, resulting in fatal injuries.

Struck against a moving object

Employee was driving a fueling truck when it turned over. It was a single vehicle accident that resulted in the death of the driver. *I will send a revised form when I get more information.

Drowning

The driver was operating a articulated truck hauling dirt from the back of the lake. Driver lost control of the vehicle for an undetermined reason and drove the vehicle over the berm and into the lake. Operator was removed from the truck and transported to local hospital then later moved to Kearney then transferred to Omaha Hospital where he died on 3/19/15.

Employee was in the process of conducting his pre-shift exam on the section when he was found by another miner. He was pinned between a fallen rib and shuttle car that had been parked in the crosscut. The incident was not witnessed. Material that dislodged from the rib appeared to have struck the foreman on the right side of his body which resulted in fatal injuries.

Struck by falling object

The 12 East longwall had mined out of seam from 3-10 shield. Shield tip to face had been roof bolted so the coal floor could be removed. The victim was standing with his back to the face. A rib struck the victim measuring 12' long by 4' wide by 1' thick. The victim was a trained and experienced longwall supervisor. The roof control and protective equipment were compliant.

The employee stated that while drilling a roof bolt hole, the drill steel started to bend, as he reached out to grab hold of the drill steel, he accidentally put up pressure on the steel which caught his left hand between the drill steel and the bolt mast. The accident was caused by the failure of the employee to drop the drill mast instead of lifting it. The ee had on his PPE.

Employee was moving top structure out of the way, stacking it against the rib. A piece of structure Employee just stacked fell over striking his right middle finger resulting in a fracture and seven (7) sutures. On April 1, 2015 Employee had surgery to repair right middle finger.

Employee directing lift operator. The forks needed to be moved together. The slide bar has pinch points. When bar moved it allows the forks around the hook. The bar did not slide, he placed one hand above the heel of the fork and one hand on attachment loop where the fork attaches to the bar. fork pushed. Fork swung back catching fingers between the fork and the outside hook

Over-exertion in lifting objects

Employee was lifting barrel into a truck, felt burning pain in abdomen/hernia.

While employee was dislodging a rock from a grizzly, a rock fell down and smashed his right index finger on mill bar.

The employee stated that while changing a belt take-up roller in the #2 dolly of 5 in the 22A belt takeup unit, he heard his right shoulder pop. The employee continued working and the incident became reportable when the employee began missing work on 04/15/2015. The accident occurred because the employee attempted to lift more than he was safely capable of lifting.

EE was checking equipment before lunch he opened the door on the hopper while it was running and put his hand in the belt area. The belt got his coat sleeves under the belt and roller and pulled his hand between the roller and frame.

Caught in, under or between a moving and a stationary object

Miner was unhooking trailer from scoop bucket when shuttle car struck trailer causing it to roll into scoop bucket crushing lower left leg.

Over-exertion in pulling or pushing objects

The injured was pushing a drill steel into the roof while roof bolting and felt pain in his abdomen. The injured was diagnosed with a hernia. His first missed day was 03/04/2015.

Caught in, under or between running or meshing objects

Employee was retrieving a product sample from a cyclone sample port. The port was plugged, so he reached his right hand into the port to attempt to unplug it when the tip of his middle finger was severed by making contact with the rotary valve above the opening.

The serviceman had finished servicing the D10N Dozer, stepped off of the push arm, and while holding onto the handhold, he stepped onto the ground while turning around and twisted his back resulting in a muscle sprain.

Fall to the walkway or working surface

Employee had a confrontation with another employee. At this time the injured employee was shoved to the ground by the other person causing him to land on right shoulder.

Struck against a moving object

While scrapping the ground, the loader made a sudden stop when the bucket hit a rock. The impact caused the head light to fall off the bracket and the operators body to jerk. Immediately thereafter, employee felt pain to his neck

Over-exertion in lifting objects

Miner had just finished helping attach a steel winch cable to an empty barge being loaded at a belt loading dock. As he was walking empty handed to the stern of the barge, he grabbed his lower right abdomen and collapsed in pain. Miner re-injured an umbilical hernia that dates back to the year 2000.

A fully trained employee was carrying a panel over to the bolter when he stepped to in a small hole in the walkway while trying to place panel on the back of the bolter and he twisted his ankle.

driller got out of drill to perform work around drill boom, turned ankle on rock on floor causing him to fall. left work at end of shift and said had no injury on following day went to doctor and found had broken bones in foot

Employee was handling a 4' x 8' piece of metal sheeting in preparation of constructing a rock box for a new transfer set up when he stumbled over top roller lying in walkway twisting his right ankle.

Employee was operating 55 jeep and struck 43 locomotive outside on the old jump track. The accident occurred because the employee wasn't paying attention. A state inspector checked the jeep after and found no compliance issues that would of resulted in accident It didn't involve job skills proficiency training or attitude. No PPE or protective devices on equipment involved.

Employee slipped and fell in bentonite and landed on his knee

Over-exertion in lifting objects

Employee stated he was shoveling wet material from under belt when he felt a pain in right shoulder. Employee did not start missing time from this accident until 05/14/2015.

Fall to the walkway or working surface

Employee was walking from MCC-1 aggregate primary to aggregate primary control tower and fell in mud and landed on left shoulder.

Scoop door became dislodged from scoop and hit mine floor. The door then hit operator in right side of hard hat. Scoop operator was taken to surface and transported to Raleigh General Hospital and diagnosed with a concussion.

Absorption of radiations, caustics, toxic and noxious substances

Employee was performing routine cleaning in the Preheat Tower with a WOMA gun. When the employee initiated the WOMA gun, hot feed blew back onto the employee causing burns to the back of his unprotected head, neck, and left ear. EE refused to wear PPE.

Struck by flying object

Employee is not sure how but a piece of metal got in his eye while driving to the shop. He had been grinding earlier and perhaps had something on his face. He was seen by an optometrist and a piece of metal was removed and a rust ring removed. He was taken off work for 2 days.

The employee stated that he climbed the stairway to install a plate on the side of the plant. The employee stated that he slipped and caught himself with his right arm and hurt his shoulder which he previously hurt at a different employer. There was no witness to verify the accident.

While mechanic was working to adjust conveyor chain the conveyor side cover plate slipped and caught EE's rt. hand between the cover plate and side box, causing a simple fracture to the tip of ee's rt. middle finger.

A PIECE OF METAL FROM A MACHINE DISCONNECTED AND STRUCK THE EMPLOYEE ON THE HARD HAT.

Struck against stationary object

Individual was using a 10 ton Simplex Jack to align a track switch. Due to poor body position when pressure was applied to the jack, it kicked out striking the individual in the right outer knee. This did not involve any compliance issues, no equipment involved. This did not involve job skills or attitude. Personal safety equipment was be used. Sprain of the LCL.

Over-exertion in pulling or pushing objects

Employee stated that he was getting up into the hauler. The first step was a little high off of the ground. He grabbed the handrail to pull himself up the ladder. When he did, he felt a sharp pain in his right shoulder. Went to ER that night to get checked. Kept off of work for the next two days.

Over-exertion in lifting objects

The miner was lifting a small pump and felt pain in his right shoulder. The miner did not start to lose time until May 8, 2015. The following were not a factor: Rules, regulations, mine equipment or systems, miner proficiency, training or attitude, protective items related to clothing or devices on equipment.

Struck by falling object

Employee was conducting a pre-shift examination and he went to lift a piece of metal from the top of the roof bolter, the metal fell onto his left foot.

Employee stepped into a parking trench and twisted his ankle. Employee received a hair-line fracture to right ankle.

Caught in, under or between a moving and a stationary object

Employee was unloading impact rollers out of a scoop bucket when the scoop operator pulled blade in on the Employee's hand which was laying on the blade causing three (3) fingers to be smashed. Employee was diagnosed with a distal fracture to his right hand and received five (5) to eight (8) temporary sutures. Employee is to follow-up with specialist

Struck by falling object

Injured Employee was assisting on a belt move in a gate section. He was installing a 2 inch round bedrail into the belt stand. Another employee was putting other end of bedrail in stand and dropped it, when it dropped, Injured Employee had right index finger caught between bedrail and stand.

Doctor indicated torn or stretched back muscle due to jarring from pot holes in county road over extended time while driving Volvo gravel hauling truck

EE alleges that as he was hauling refuse down the ramp the truck began to slide and the truck started bouncing heavily in some mud when he hit his head on the inside of truck cab. The co. does not believe that there was a reportable accident for the reason the co. does not believe the EE rec. any injuries. This form is being submitted to abate cit.#9062260

Employee was driving CAT 773 Coal Truck when he was empty going down a ramp he hit a rough place in road causing him lower back pain. He finished his shift and did not miss a scheduled work day till 3/30/2015

Mounting steel plate to conveyor belt on the shop floor. EE lifted a 4'x 8' steel plate off the floor to stand it up and slide pallet jack under it, EE felt some strain. Later that day, while mounting 2nd steel plate & moving plate by hand with pry bar, EE felt a sharp pain in lower back.

Employee's right arm was caught between the feeder and the feeder trailing cable when he was attempting to trip the machine's service breaker. The cable had been caught by a rock in the feeder and became tight at the point he reached for the breaker switch. The injury resulted in a fracture to the radius and ulna bones.

The person was attempting to remove a ""stuck"" bucket on a skid steer loader with two other people when it came free, dropping down to the ground and crushing his left foot. Three bones in his foot are broken and surgery is expected to be needed to repair according to the ER MD.

EE was attempting to recover a valve and small piece of 4"" HDPE pipe from under a rock. The EE attempted to lift a large rock with his left hand and pull the pipe with his right hand. EE was moving the forks in on the Lull, in order to do this they have to raise the boom up & then tilt it forward so the forks will slide. As the Lull operator was tilting the forks forward, the forks & the rod fell out & the forks hit EE on top portion of his thigh. He was able to finish job before he went to dr. Employee was welding on chute in plant when a piece of slag fell and entered his boot burning a place on top of his foot. He finished his shift and did not miss a scheduled work day till 4/1/2015.

The accident occurred because the employee was not observant of the mine floor conditions in the area he was walking. A contributing factor may have been the new boots he was wearing for the first time on this day.

A fully trained employee was moving rockdust hose to second setup from 46 wall to 41 wall pulling hose along beltline for a 1/2 block long. Employee struggled to pull hose and the employee strained his lower left back.

EE was clearing dust from photo eye the Fuji Robot. After cleaning photo ee noticed a bag that was not completely at the end of the line which would cause a problem when robot picked up bag. He then proceeded to step down to move the bag to the appropriate place, but when he took the 1st step with his right foot he stepped on a bolt sticking out of the ground, which he did not see.

The ride has a spare tire attached to the battery lid cover and when he moved the spare tire, he felt a muscle strain in his back. The employee finished his shift but he went to the doctor on 3-15-15. He was placed on light duty for 1 week with a return to full duty on 3-23-15.

Bates Contractor: During an attempted retrieval of an immobilized continuous miner, a 2"" connecting clevis pin broke away from the two bars it was attached to and the pin passed through the protective wrapping and struck the Bates Contractor in the chest. He was approx. 25' away in the intersection out by.

Employee was lifting 55# buckets of refractory into a cement mixer. While lifting a bucket up to dump in the mixer, he felt a pain on his left side between his mid side and back. Employee was sent to Dr and placed on restricted duty (no lift, push, pull > 10#) until recheck on 3/17. Released to reg duty 3/17.

Miner was climbing down from loader. He had a water bottle in one hand. He failed to maintain 3 points of contact as trained. He fell from the 2nd from bottom step onto concrete pad.

Third Party Blasting employee bumped into another employee from his company and fell on our property. INJURY WAS NOT REPORTED TO TILCON until today March 23rd, 2015 at 12pm. 3rd party company involved: Kemek Injuries sustained: landed on his hand to break his fall and later felt discomfort in his wrist and reported it to his supervisor. The outcome was a fracture to his wrist.

Employee was working on a moisture analyzer which had a small amount of water that overheated and sprayed out a blast of steam and hit him causing burn to his abdomen.

EE was prying on a crusher housing to set it back in place. He was standing on a wooded board covering a work hole used for maintenance on the crusher. The board broke and he fell through the hole catching himself with his arms chest high with his left leg/foot landing on an I-beam. The employee sustained a sprained left shoulder, leg/knee and ankle.

While operating chain hoist hook to pull a piece of worn steel (appx 150 lbs) out of a headbox chute, the hook end attached to anchor point rolled over once tension was applied by EE, pinched left index finger tip between the chain hoist and anchor point. Designated person working with EE assisted in relieving tension off of chain hoist hook allowing him to remove his finger.

EE was attempting to open the quarry office door when the handle broke in his hand. When this happened the sharp edges that were created in the break cut the right hand of the EE causing a laceration that required 6 stitches. The EE was placed on modified duty.

He was using a slate bar to pry the belt structure into place and the bar slipped, causing him to lose his balance and fall to the ground. This resulted in a left knee and shoulder strain.

Employee was washing under the supports under the KVS scrubber. Employee discovered a hole through his right boot. Puddle water got in his boot and contacted the top part of his right foot. He noticed irritation a few minutes later.

Pan conveyor fell after air tugger was pulled from its anchors.

Over-exertion in pulling or pushing objects

Employee was replacing some floor grating in the water tower building. While doing this task he had to pull some of the grating up by hand. He felt a pull in his left shoulder while pulling up the grating

Fall to the walkway or working surface

The Employee had dismounted his front end loader and was standing next to the loader when he slipped. The slip and fall accident was due to the Ice that he was standing on next to the loader

Fall onto or against objects

Employee descended the stairs and stepped to the ground, turned, stepped on ice, fell extending his left hand to grab the handrail. He instead struck a crusher wear liner laying on the ground lacerating the palm of his left hand. He was wearing all required ppe including leather gloves. He received 5 inner and 14 outer sutures.

Struck against stationary object

Employee hit his left thumb while dumping a hopper.

Struck by powered moving object

An employee sustained a fractured rib on his right side after being contacted by the fork frame of a Tele-Handler fork lift while moving a pallet out of a connex container.

Injured was Roof bolting and was using split steels. When injured raised pot to seat the steel into the chuck the steel caught the corner of chuck, causing a buckle to the steel and striking the victim on the left hand causing a fracture to the 2nd Metacarpal.

Operator complain of pain in his wrist while operating shovel and loader.

Caught in, under or between a moving and a stationary object

Injured was setting a timber. As he moved the timber his thumb was caught between the timber and mine roof.

Fall to the walkway or working surface

EE getting off the cage underground fell hitting elbow on ground. Fracture of bone in elbow.
