Accident type, without injuries

Hitachi EX3600 excavator was loading trucks when the engine/hydraulic compartments on the rear of machine caught fire. Operator exited the machine safely and fire departments were called to help extinguish the fire.

A roof fall occurred at spad #590 on #3 belt. Ventilation nor equipment was not impeded but passage was blocked. The fall measured 20' wide by 60' long by 5' thick.

A roof fall occurred at crosscut No. 111 in No. 5 entry of the 7L panel. It was limited to the intersection and will not be loaded out. The unit that was inby that location has been moved outby the fall to a new panel.

Night shift crews were entering the underground at the start of shift through portal four. The crew sensed a bitter acid taste as they entered. The crew immediately retreated out of the mine and contacted management. The mine was left to ventilate, MSHA 1-800 number was called. Mine rescue team determined a sulfide dust explosion occurred during day shift blasting.

On Thursday September 27, 2018 at approximately 6:20pm a roof fall occurred on the J-1 section in the number 1 entry on the J-1 longwall set up. The roof fall is approximately 30' long by 21' wide by 9' high.

The elevator at the portal went down at 4:56pm due to a faulty gate switch. The switch was replaced and the elevator was put back in service at 7:15pm.

IGNITION - Occurred on No 2 longwall while the shearer was cutting coal and rock mining toward the tailgate. Ignition lasted approx. 15 to 20 seconds and was extinguished by fire extinguisher.

An unintentional non-injury roof fall was discovered while doing weekly exam, in the #1 entry of the #1 east mains return, approximately 130 feet in-by the mine portal. The fall was reported to MSHA Hotline, the fall will be cleaned up and re-supported. The fall is 16' wide and 4' high and 4' to 5' long.

An unplanned roof fall was discovered at 6.5 block in the #4 Entry of 6 North Parallel. The fall was approximately 16' wide, 50' long and 4' high. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

An Unintentional roof fall Occurred on 4 North Mains Brk 34, In the #2 left crosscut approximately 20' North of SS 16267. The fall occurred in the area of the old 3 Left Panel at the #4 seal. This fall was discovered by the weekly examiner @ approximately 2:30 PM

A roof fall was discovered on 2nd South Mains at wall #80. The fall was 25 ft long, 25 ft wide and 6 ft high and impaired travel in the intake travel way. The area was mined in 2011 and was supported with 6 ft resin bolts. The area appeared to become unstable over time due to a localized geological change from shale to a shale/sand rock lamination.

The Crabapple hoist was taken out of service for an extended period of time due to damage to the hoist rope. During regular inspection a small kink,or deformation, was found on the hoist rope approx. 15 ft from the hoist car. The cable was cut off and re-terminated.

Unintentional roof falls occurred in 3 East Mains 10 Wall #4 Entry Intersection and 11 Wall 3-4 Crosscut. There were no injuries as a result of this incident. Conditions contributing to the roof fall is humidity in the area and adverse roof conditions.

Smoke was noted in the West Pit ramp 35 area. The smoke could not be extinguished within 30 minutes so MSHA was notified. Measures were taken to protect employees and a plan was formulated to dump over the fire, but would wait for day shift so trucks dumping would have good visibility. The smoke was extinguished by 10am 09/23/2018.

The a phase 13.8kV primary with 4160V secondary transformer shorted in the coils. This caused a small fire which was extinguished within minutes. The a phase and b phase coils were destroyed.

IGNITION - Occurred on No. 2 Longwall while the Joy shearer was cutting and came in contact with a pumpable crib, the wire became entangled around the drum causing a frictional ignition with the drum at shield 175, which was extinguished by water and fire extinguisher.

At approx. 12:40 pm a roof fall was discovered in the 6 North Left Return at 256 1/2 blk. The fall is approx. 12 ft. wide x 75 ft. long x 8 ft. high. There were no injuries, impaired ventilation or damage to equipment as a result of this fall. Surrounding areas are being supported and the affected area is no longer accessible.

At 5:03PM, the surface supervisor received a call from a 2nd shift employee in Pit #11 of smoke coming from the 1-54 Shovel and it could potentially be on fire. The surface supervisor immediately drove back to the pit and found the 1-54 Shovel on fire.

The 12 Silicon Controlled Rectifiers for the #3 hoist failed due to age and usage over time. New units were on hand that we were able to replace these with. It took time troubleshooting the hoists electrical fault to determine the SCRs were the issue.

Roof Fall occurred in the 7 Seam B Return. The Fall was located in B entry at 5+15. The dimensions are 40 feet long, 18 feet wide by 7 feet high.

During the lighting of the K-1 Burner we had a buildup of gas causing an unplanned Ignition of that gas.

Roof fall occurred in the First North Main #1 entry xcut 29. The fall was L16' x W18' x T8' and was supported with 6' fully grouted roof bolts. The roof fall will not be cleaned up. Date of development was 9/2015.

An unintentional roof fall occurred in #2 entry, at 2460' station of the 3rd South. The fall was approximately 80' long, 18' wide and 6' thick. No one was injured, it did not block an escape way or affect ventilation.

Due to a geological deformation of the roof strata on 5 right a roof fall occurred. The roof fell in the #3 beltline entry. 20 feet inby s/s 5972 continuing inby 1 x-cut to 50 feet outby s/s 5987 and also in the #4 primary escape way entry 50 feet inby s/s 5975 continuing 1 x-cut to s/s 5986.

An unplanned roof fall occurred at 4:35p.m. in the 1st East, panel #1 at spad 3+00, the roof fall was above anchorage, and continued into adjacent intersections. This is a worked-out area, no ventilation or escape way effected. Floor to roof support were installed and area dangered off. This roof fall will not be loaded out.

Power bumped and caused the Rodeo shaft to be down for more than thirty minutes. No one was on the cage at the time of power bump. Power bump was caused by outside source and beyond the control of Goldstrike UG. A courtesy call to the MSHA hotline was made with a follow up call to Gary Hebal the Elko field office supervisor.

A non-injury roof fall at the #3 mine portal, the fall is approximately 16 feet wide, 5 feet long, and 4'-6' high. The fall was reported to MSHA hotline the fall will be cleaned up and re-supported.

On 09/17/2018, The superintendent came into the plant after Hurricane Florence had subsided. EE noticed that the Railroad pond impoundment had breached. This was at 6:57 am. We immediately called the MSHA hotline and reported the failure. There were no injuries due to the failure of the impoundment.

An Examiner was traveling the 1st North Submain 4 belt Intake, examiner was looking for a broken phone line and found a roof fall in the #7 and #8 entry. The Roof Fall was immediately called into the 1-800 MSHA Hotline.

The Sparta elevator became inoperable on the bottom. There were no personnel on the elevator. The clutch wheels for the bottom door was out of alignment and needed repaired.

An unplanned roof fall was discovered at 14-38 crosscut on the mainline haulage. The fall is approximately 10' high, 16' wide and 25' long. The fall will be cleaned and re-supported.

A roof fall occurred in the 9 seam in the 1st Northeast submain panel 1 at xcut 9 between 8 and 9 entries. The roof fall will not be cleaned up. The roof was supported with 6' fully grouted bolts and 8' megabolts as supplemental. Date of development 10/2017.

Caused by a geological anomaly of the roof strata in the #4 primary escape way entry of main D a roof fall occurred. The roof fell at s/s 2897 proceeding inby one crosscut.

An unintentional roof fall has occurred on the M12 at the 32nd Xc of the number 4 entry. The area was in the process of being re-supported when the failure occurred the fall was approximately 35' x 40' x 12'. A site specific plan was submitted and the area is having additional support installed.

During the skip re-alignment process on 9/10, the hoistman took the hoist out of safety mode to reset the skip but forgot to place the hoist back into safety mode for the 2nd skip's alignment. Thus, leading to the west skip over-traveling through the over-wind safety circuit and the track limit safety circuit causing damage to the cappel and the skip itself.

Escape way hoist in Shaft 1 was found to be inoperable at 3:11 a.m. A line filter had shorted out on the hoist safety system. The hoist was returned to service at 3:45 a.m.

At approximately 12:15 pm it was reported that there was a hotspot on the raw coal stockpile. At no time were there visible flames. We are following an in-house action plan to monitor and control the situation. If there are any changes in the situation both State and Federal Agencies will be notified.

At approximately 5:55 p.m. it was called out that there was high methane readings in the old Dog Leg 2 panel. All power was de-energized, employees were evacuated, and both agencies were notified. No one was injured.

A brush fire started on the evening of 9/9/18. The fire was most likely caused by lightning as there are no mining activities near the point of ignition. This brush fire is being reported as it took longer than 30 minutes to extinguish the fire from the time it was reported. No one was injured while extinguishing the fire.

On 9-7-18 a rib roll occurred obstructing travel on the mainline tract at 18+00. The rib roll took out cribs and false top beams. the area was 15' long and 6' high. the cause of the rib roll was weathering and additional support will be installed in areas that show weathering.

An unplanned roof fall occurred on the South Mains Track at 11 crosscut. The size of the fall was approximately 75 feet long x 18 feet wide x 5-1/2 to 6 feet high.

Elevator suddenly stopped between floors trapping employee in the elevator. Employee used the phone to call for help. MEI was called. MSHA was notified of the incident in a timely manner. Employee was freed from the elevator after approximately 49 minutes. MSHA was notified with in 30 minutes of the incident.

Hoist man moved conveyance without a release bell from cagers working on 4600 #3 Station. One cager was inside cage when it was moved and other cager was on station.

Three employees were on the 14ft. skip being lowered in the mine. The skip tripped. A maintenance supervisor was called to the area and began troubleshooting the problem. Radio contact was made and the 3 employees were fine. They were lowered in an egress mode. They exited the skip and all were in good health.

An unplanned ignition of coal dust occurred in the #2 kiln coal mill system fan as a result of an inboard bearing failure. The bearing failure created the ignition source. The rupture vent opened as it should and there were no personnel injuries or equipment damages.

An unplanned roof fall was discovered at 16-19 between the track and neutral entries. The fall is approximately 20' long, 16' wide and 10' high. The area has been posted and dangered off.

A roof fall above the anchorage point occurred on the old mainline at 306+00 the fall approximately 30' long 25' wide and the height is undetermined. the cause of the fall was due to weathering and additional support will be installed in areas that show weathering to prevent re-occurrence.

An unplanned roof fall was discovered at 17-04 on the mainline belt. The fall is approximately 17' wide, 20' long and 15' high. The mine is in the process of cleaning and re-supporting the area.

A roof fall was found in the #7 entry outby 2 section near spad 11289. The fall was called in to both state and federal agencies. No one was injured.

The Sparta elevator became inoperable at 7:05pm. After investigation it was determined that a broken wire from the junction box to the remote pendant for the emergency stop caused the problem.

At approx. 8:33 am, received notification that the 7 North Hoist was not cycling properly and was being removed from service. Schindler Elevator Service was called to troubleshoot/repair the hoist. A ""go"" switch was found to be faulty and was replaced. The hoist was back in service at approx. 1:00 pm.

The CM had backed out of the number one entry and began tramming to the number two entry when ee noticed the front MBC on the attached system did not pull ahead. At that time, the victim was found at the MBC suffering from crushing injuries. This resulted in a fatality.

Contact with hot objects or substances

The IE was hauling overburden in a Cat 793 end dump to a fill area. The front area on the end dump caught fire. The IE parked the truck on the fill and exited the end dump. The IE sustained multiple burn injuries as a result of the exit. IE passed away on 09/12/18. Ultimate cause of death is pending the coroner's report.

The employee was operating the unit scoop. EE got out of the scoop for some unknown reason, the scoop started to roll. While trying to get back in the scoop, EE's right foot was caught between the coal rib and the scoop deck, resulting in a partial amputation of EE's right foot.

Struck by falling object

Trailer gate was chained open to the right side of trailer. Employee was standing behind trailer washing trailer out. Chain holding trailer gate gave way and swung down striking employee's right ankle and foot.

While opening the flop gates on an Allis screen to control the rock overflow employee got the tip of the left ring finger below the fingernail but above the last knuckle closest to the end of the finger smashed off when the employee left hand testing below the flop gate handle

While repairing a seat in a drill, a craftsman's hand was caught between the door and frame when a high gust of wind caught it and slammed shut, impacting fingers on the left hand. EE's left, middle and ring finger had the pads on fingers above the first knuckle tore off, and small laceration to tip of index finger.

Employee was helping change scoop batteries when EE's finger got caught between the scoop tire and fender.

Caught in, under or between running or meshing objects

The individual was attempting to clear a dust chute and mistakenly made contact with a rotary feeder. The end digits on the index and middle fingers on the left hand where crushed.

Struck by falling object

Employee was on-shifting the 9N#3 entry from the mouth to the 25 break t-car. The employee had gone through the first check curtain located just inby #4 break and was headed towards the 2nd check curtain when a rock, approximately 3' x 3' x 1' thick, fell from the top and struck the employee on the hard hat driving ee to the ground.

Employee pre-spotted Haul Truck #29 at Shovel 3. When the operator of Shovel 3 went to clean up material in the Shovel pit, the dipper caught the dove tail, raising the rear of the truck.

Caught in, under or between a moving and a stationary object

Employee was helping hoist a gearbox in place when a chain slipped as the bolts were being installed to secure the reducer causing it to catch employee's hand pinching it, requiring sutures.

Over-exertion in lifting objects

Employee was in the process of installing a bottom roller on the parallel C-Belt. While lifting the roller EE felt a pop in the right shoulder. Employee was referred to a specialist on 10/19/2018 for an MRI and was placed off work.

Caught in, under or between a moving and a stationary object

The employee was prying a cover off of a stage loader crusher, that was being held up by come-a-longs. When the come-a-longs got the full weight of the cover, the come-a-long came loose from the top and the employee was caught between the crusher cover and a pile of cable laying on the pavement behind EE. The employee was wearing all the appropriate PPE.

Caught in, under or between a moving and a stationary object

Employee was part of a crew advancing the belt in the 6 Mains West section. Employee was re-hooking a wire rope to a scoop to pull the belt onto the structure when the wire rope inadvertently tightened when the belt grabs slipped pinching the EE's right pinky finger causing an open fracture and laceration to the finger.

Struck against stationary object

Working in a manlift in tight quarters while changing a roller bearing, employee turned and struck temple on a piece of angle iron.

Struck by rolling or sliding object

Repairing a steel plate in the feeder they had the backhoe bucket sitting on the plate holding it in place so they could weld it in place. The metal plate shifted & caused the bucket to slide forward hitting the employee in the leg. The employee's leg was bruised. We had employee go to the emergency room as a precaution to eliminate any serious damage. There was no major damage

Fall onto or against objects

Employee was walking down the stairs checking the belt in the raw coal tunnel at the Preparation Plant when their foot slipped, causing them to catch themselves on the handrail and caused injury to right upper arm.

Over-exertion in pulling or pushing objects

Employee was hosing floors. When moving the hose for positioning, employee felt a popping in the lower stomach area. Employee felt a bulge below the belly button.

Absorption of radiations, caustics, toxic and noxious substances

EE was climbing down a ladder when they felt something enter their right eye. EE was wearing all required PPE. EE immediately reported the issue and began flushing eye with saline eye wash solution. After a couple of hours of eye washing, Site manager suggested that the EE go to the clinic to get checked out. EE was prescribed Prednisolone eye wash solution.

Over-exertion in lifting objects

IE was loading bolter with 3' glue when IE felt right shoulder pop.

Absorption of radiations, caustics, toxic and noxious substances

Employee received chemical burns to left foot after hot material sloughed onto lower leg while cleaning in the Dog House. Material entered into employees left boot. Employee continued to work then felt discomfort after the end of shift. Employee then sought medical treatment.

Struck by falling object

Lifting a rock slab, rock slab slipped falling on and crushing left ankle.

Bodily reaction, (Not Elsewhere Classified)

While walking in Bath house on concrete floor at the start of employee shift the employee stepped and pushed off with right foot felt pop in right heel. Employee did not start missing time from this injury until 10/03/18.

Fall to the walkway or working surface

Employee was dragging a load of wire mesh to roof bolter. EE was dragging it at a rapid pace, leaning forward and struck head pin/pin board knocking ee to knees, ee nose began to bleed, and ee was dizzy/light headed and began to feel drunk like.

Struck against stationary object

This report is being submitted under protest. The operator does not believe that an event occurred that led to a personal injury: The employee was operating a Fairchild scoop. Employee stated that employee ran over something in the roadway that caused the scoop to throw employee up in to the mine roof, striking employee's head.

Caught in, under or between (Not Elsewhere Classified)

The employee was carrying a box of SCSR's around a scoop in the LOC of the longwall section. The employee tripped and fell down while employee's hand was still in the handle of the SCSR box. The employee fractured the middle and ring fingers on right hand, and also sustained lacerations on those fingers. The employee was wearing all the appropriate PPE.

Fall to the walkway or working surface

EE was helping to lift an empty water pipe, which was braced on a wooden block with a prybar between the pipe & block. As EE lifted the pipe, another employee kicked out the wooden block, causing the prybar to fall. EE stumbled and fell while trying to catch the falling prybar.

Caught in, under or between a moving and a stationary object

Employee was greasing the tile saw and EE's feet slipped out from underneath EE. EE slid between the tile saw and the wall, EE's left arm was up against the wall at a bad angle and pain shot through it.

Fall from machine

EE was climbing into a Dozer when EE went to grab the door handle, EE slipped and fell 7 feet onto EE's back.

Contact with hot objects or substances

During lubrication process the miner made contact with hot grease at mills number 3.

Fall onto or against objects

While helping another employee who had fallen, the injured employee fell and struck knee against a block, resulting in a laceration.

Over-exertion in lifting objects

A fully trained miner was lifting a shuttle car tire when ee felt pain in left shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

Standing in belt entry, took one step backwards and pivoted on left foot, felt left knee pop.

Struck against a moving object

Tension spring came off of brake pedal. While tramming to park equipment, pedal fell to floor causing brakes to fully engage. Employee's upper body thrown forward while seat belt kept lower body stationary creating whiplash effect.

Struck against a moving object

EE reported hitting the headliner of a pickup truck with hardhat causing pain in neck and back. Employee was front seat passenger when the pickup made contact with the end portion of a dirt berm located along tie down area. Employee was assessed by on-site medical responders, and then brought to hospital for further evaluation where medical treatment was provided.

Miner strained lower back when EE was laying down on back side shoveling out from under the jaw crusher.

Caught in, under or between a moving and a stationary object

Employee was releasing a belt clamp after performing belt maintenance on a section belt. As the belt was being released it moved through the clamp and pinched employee's right index finger between the clamp and a bottom roller.

Employee got their glove caught between steel and wrench while bolting. This caused their hand to be twisted.

EE was on a forklift getting a roll of poly bags down to take to the packaging line. Prior to doing this the teammate must take the plastic wrap and cardboard off of roll and lay to the side. When EE came back to clean up the trash, got off the lift and squatted to pick up the plastic and felt a pop or something pull in lower back.

EMPLOYEE WAS SHOVELING SALT ONTO CONVEYOR BELT WHEN EE FELT PAIN IN LOWER BACK.

The employee was picking up a cinder block, and as ee turned, ee felt a pain in back.

Employee was operating a Cat 793D haul truck and was being loaded by a L1400 Letourneau loader. The loader put the third bucket in the truck when a rock rolled to the right side of the truck box and caused the truck to rock which caused the employee pain in the neck area. EE was operating a 20 ton motor as a trail motor when two empty supply cars went off track in front of EE. EE could not get stopped and hurt left shoulder when the motor struck the empty supply cars. The ee continued to work till EE went off for shoulder surgery on 02/06/2019.

Injured employee was checking the charge level on scoop batteries. As EE was checking the batteries, the rib rolled out behind EE striking EE in the back and shoving EE into the scoop body and down to the mine floor. Employee has contusions and a right shoulder strain/sprain. Employee has proper training & experience and didn't violate any rules or regulations.

EE was roof Bolting, as EE was dropping the bolter head, the head struck the second drill steel EE was holding in right hand. The second drill steel punctured EE's right thigh. EE returned to full duty on 02/22/2019. EE went back off work 2/17/2020. There was not a Reoccurrence of injury.

An MRI was performed on 10/12/18 and revealed a tear of medial meniscus. IE followed up OISO on 10/22/18 at which time a non-weight bearing restriction was applied becoming lost time.

As the bolter was being moved around the corner the ATRS pad flipped up. IE used the palm of left hand to flip the ATRS pad into the normal position. While they flipped the pad a rock fell out of the roof between the hog panel and the rib, striking them on the middle, ring and little fingers causing fractures to the tip & avulsions to ring and little fingers.

EE was roof bolting, as ee was dropping the bolter head, the head struck the second drill steel ee was holding in right hand. The second drill steel punctured right thigh.

IE was performing a pre-trip inspection on the end dump IE was assigned to operate. As IE was walking around the end dump IE slipped in mud and broke fall with left arm. IE's left arm was immobilized by a doctor and IE was subsequently unable to return to work.

Employee was working on number 1 tray in unit 345 when caustic sprayed into employees eyes. Full proper PPE was on at the time of the incident.

2 employees were installing a new conveyor stand. They were attempting to line up the bolt holes to secure the new stand to the conveyor. 1 ee thought that the other ee said push (ee had not) EE pushed the bottom of the conveyor stand at the same time the top shifted and came down on ee finger tips, that were between the conveyor and new stand.

Employees right knee buckled, causing ee to loose balance and fell into an extension ladder hitting ee rib cage on tight side.

A miner sustained an injury to the back of their right hand when their glove became caught in a conveyor and pulled their hand in. Damage and removal of tissue on the back of the miner's right hand was caused by this incident.

The employee opened a sample port valve to determine if a hydrated lime blockage had cleared the elevator. While the valve was open the blockage broke free sending material out the valve and onto the employee. The employees was wearing safety glasses but the material got behind the foam seal of the glasses and caused injury around and to employee's eyes.

Steel bracket was on top of the bucket, when the bucket was tilted it fell off and struck the technician on the shoulder, and arm.

Employee had their hand on the drill pot of the roof bolter checking the suction and a rock fell and struck their hand. Employee fractured their right middle finger.

Employee was prespotted at a shovel and the operator states the dipper made contact because the truck was too close. The operator said they were fine through the shift and woke up with back pain before the start of their following shift.

Employee was pulling a 1 1/2"" water line in the pit area of the slope belt when they felt a pain in the lower back.

Operator of All-terrain lift did not see employee in blind spot and contacted employee's leg while moving all-terrain lift pinning the employee's leg between the all-terrain lift and a collection box.

IE started missing on 9/25/18 due to this incident.

EE notified supervisor at end of shift their lt. eye was irritated. EE claimed that during safety meeting at start of shift something got into eye & over the shift caused the irritation. Supv. offered to have EE seen by mine rescue & get eye flushed. EE refused treatment at mine; however, sought med. attn. after shift. There was nothing in eye to remove but cornea was scratched.

Employee was walking down the steps and stepped onto coal accumulations when EE slipped and fell landing on EE's back.

IE was traveling North pulling supplies with a locomotive. As ee was approaching the 18E switch, the locomotive went East into the switch because someone had left it open. IE's Locomotive struck a parked manbus that was unoccupied. IE starting missing on 9/26/18 due to this incident.

The employee was working in a belt line recovering belt structure and waterline when the employee felt pain in upper back. The employee was wearing all the appropriate PPE and did not violate any state or federal laws.

Company electrician was lifting a 40lb roll of wire into a van from the ground and injured their shoulder. Electrician did not lose time at the time of the incident, but needed surgery to repair the injury on 11/28/2018.

EE continued to work and while off during Christmas break, EE had knee surgery. After working over 6 months, the leg has not improved, so EE is now off to have total replacement, as of July 11, 2019.

Employee stated EE abruptly exited a scoop deck and bumped left side against the scoop deck frame and injured left side rib cage. Employee sought medical treatment after shift and was diagnosed with a fractured rib. Employee was taken off work.

While track bolting, employee was changing steels and the mast came down on their right foot. Employee was diagnosed with a fracture right big toe.

Employee was bolting on the opposite of the operator's side of a double boom bolter. They had finished installing a row of bolts and ee had returned to the side of the bolter to get a drink of water. The operator moved the bolter from the inch controls. The other operator's right foot was too close to the wheel and got toes under the wheel.

The victim was loading rock dust when they threw a bag of dust. They hurt their right shoulder.

EE was observing mechanics repair the mesh handling tray jacks on a walk-through bolter. As the mechanics re-positioned the tray, the safety chain broke causing the tray to strike EE on the right shoulder & upper back. EE suffered 2 fractured ribs and ee began missing time on 09/17/2018.

Employee reached across a stack of metal roof straps to grab two additional roof straps. As EE grabbed them and pulled them toward self, EE drug the large stack off onto EE's right knee and foot. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

A fully trained miner was removing a piece of water line when the line fell striking EE's knee and foot. The employee did not begin to miss work until 10/03/2018. The injury was not the result of any compliance issues nor were any protective guards involved.

The employee was fully released to duty, and didn't start losing time until 9-24-2018. The employee suffered a hand injury at home after the eye injury and was off from 09-17-2018 until 9-21-2018.

Employee picked up a belt roller and felt a pain in groin. EE did not seek medical treatment and worked all scheduled shift until being taken off of work on 2/1/19 to repair a hernia.

Employee was in the process of changing out some fasteners on conveyor belt, when they pulled out wooden board they felt lower back pain.

Employee twisted back when stepping down out of the bulk truck. Later Shoveling dirt when putting off the shoe, aggravated it even more.

While welding, slag fell on top of EE's steel toed boot and burned through the top of the shoe. The EE was taken to the clinic and diagnosed with three small first degree burns. The wounds were cleaned thoroughly, and EE was given instructions for care and released with no restrictions or follow-up. Became reportable on 9/14/2018 after an infection was diagnosed.

While working on a tail pulley (large, heavy, metal part) the pulley rolled off of a sawhorse and employee reached to catch it. Employee's back was hurt while catching/holding/lifting the part.

Employee reported that employee was cleaning under C-1 belt on the MCC side and when employee went to get up, went blank. Employee noted that employee did not know if employee raised up and hit head on the structure or if employee stood up and slipped and fell and hit head. Employee stated that employee was wearing hard hat.

IE was lifting a spring assist ramp of a utility trailer and felt some discomfort in shoulder. IE was evaluated by an ortho where an MRI was ordered. The MRI was read on 9-28-2018 where a rotator cuff tear was diagnosed with corrective surgery for Oct 23.

The excavator was being moved across the floor of the quarry with the windshield open when the windshield came unlatched and came down pinching operators hand in between the hand controls and the windshield frame. EE was taken to the hospital to get x-rays and receive stitches. The x-rays showed no fracture and ee received 4 stitches.

Ee was walking through thick slurry material in a containment area. When ee went to pull foot out ee felt a pop in low back. EE was diagnosed with a low back strain. Put off work pending a follow-up on 09/17/18. EE missed one scheduled shift.

Injured employee was hanging continuous miner cable in the #6 entry when EE heard a pop in the right elbow which resulted in a sprain / strain type injury and lost work days.

Injured was loading trash in a trailer. Foot was under the rub rail on the trailer. The trailer was pulled forward causing a small fracture to EE foot.

Employee was mounting #10 Jeep when employee slipped and struck right knee on the tram lever of the jeep. The employee was not removed from work until 09/14/2018 and began to miss work on 09/14/2018.

Contract miner was injured while cutting rib mesh away from a bolt. The employee was outby the stageloader on the off-walkway side. Miner was struck in the arm and hand by a piece of rock that had fallen from behind the mesh. The miner received two broken bones in the hand requiring medical treatment.

Was walking along low belt,stepped on a rock or on rail of low belt and twisted left leg resulting in fracture.

A fully trained miner was picking up a block when the miner felt a pain in lower back. The injury was not the result of any compliance issues nor were any protective guards involved.

Installing counterweight pulley on field conveyor. Tele-handler and come-along holding pulley on either side of conveyor. Come-along cable snapped. Pulley swung back and struck middle finger of right hand.

Employee was walking through shop stepped from gravel to concrete causing self to slip and fall injuring left foot. Incident resulted in a fracture of the left big toe/foot.

Hired mechanic was using porta power to push out the tail pulley of the impactor. The wood snapped and sent the tail pulley back and EE's hand got stuck/crushed in the impactor. The machine was reset and the mechanic went to the hospital.

While lifting Miner cable from cable hanger employee felt pain in right shoulder. Injured was diagnosed on 10/10/18 for a torn rotator cuff.

Employee was cleaning around grinding mill #5 when mill puffed a cloud of dust into the air due to back pressure in the mill system. Natural barium sulfate (barite) material got into employee's eyes and back. Employee flushed eyes with water and eye saline. Employee was treated for eye abrasion and prescribed for eye drops.

Employee was bending over to put a steel cover on a pontoon when EE felt a sharp pain in EE's lower back.

Employee was walking on a downward grade on loose ballast, (small rocks along railroad tracks) the ballast slid causing the employee to fall backwards, as employee was falling employee put right arm out to brake fall, impacting hand and breaking right wrist.

Employee was pulling pins during retraction. When the unlatching jacks raised the latched stuck causing the push beam to raise appx. 4 inches. When the latches came loose the beam slammed down.

Contractor was installing liner on the blade of dozer when a bolt broke that was supporting the metal plate. The plate dropped striking the contractor on the left foot. Result was a bruising and a fracture of minor bone in the left foot.

EE was able to walk on the foot. Told supv. that they ""took a digger"". After seeking treatment, determined to be a R ankle sprain. We accommodated light duty and there was no lost time.

Employee was sitting in the cab of Company #443 haul truck a 785 Caterpillar haul truck, being loaded with dirt. An unseen rock in the bucket struck the truck bed jolting the employee.

The injured was using a set of 3/4 ton chain tuggets to secure a load on EE's supply trailer when the pin holding the hook in the chain failed allowing the device to strike EE on the left knee resulting in a bruise.

Employee was pushing a wheel barrel of rock during cleanup and felt pain in neck and shoulder.

A piece of a cone shaped plate was being lifted into place. The plate started to tip over and the supervisor said to let it fall because it was still attached to the chain fall. While the supervisor was talking, EE reached out to grab the chain and cut left middle finger on top of the plate. EE had not yet put gloves on.

Employee was walking on ground level pathway when the employee slipped on surface and fell to ground. Employee straightened left leg out and felt pop. Employee worked rest of shift. Was seen by doctor approximately 7:00pm that night and was diagnosed with a dislocated left patella. Put in brace and given prescription and was released to light duty.

Over-exertion in lifting objects

On 9/28/18 employee reported left shoulder pain. On 9/11/18 Employee felt tweak in shoulder after attempting to lift the outside ring for pellet die into place. Employee did not report immediately as EE thought it was just a minor strain or bursitis that would improve. Pain got worse over time. Employee was seen at the clinic on 10/3 and was put on restricted duty.

Rubbed or abraded

While training a conveyor belt, using a hammer to adjust return roller, employee missed roller frame and the hammer was caught between the belt and roller, pulling hand against the belt, causing a friction burn on the top of left hand.

Employee was assigned to load shot rock and #2 base into CAT 775 haul trucks. Employee started feeling discomfort in the lower back around mid-morning, it was not until EE got out of the loader to use the bathroom that EE felt a more intense pain coming from the lower right side of EE's back. This was about 10:00 AM.

Operator was moving the manlift basket up to do maintenance on the gator screen shoot. As ee was going up, ee didn't see the 2ft long by 1ft wide metal slanted roof. EE hand was on the railing of the manlift basket when ee hand became pinched between the metal and the rail. EE pulled hand out causing the laceration. EE was brought to the walk-in clinic and treated for injury.

Employee was cutting vegetation with lopping sheers. Employee twisted their body and strained their left ankle.

EE was removing a roller on UG belt conveyor, right hand was on top of the structure rail. As EE was pushing the roller with EE's left hand, the roller slipped & rolled onto EE's little finger of EE's right hand, pinching/cutting the tip of the finger. EE wearing gloves at the time of injury. EE taken to clinic & received 4 stitches, antibiotic, pain med & placed on restrictions.

Employee, along with 3 others were repairing splice on Thayer belt feeder. While removing tail pulley shaft, tail pulley shifted trapping left ring finger between tail pulley drum and bearing assembly. Employee's left ring finger fractured. Restricted duty - no lost time.

An employee reported that ee felt pain on the top of foot from walking. The following day, 9/10/2018, the employee was given restrictions during the medical follow-up appointment.

EE had stopped and locked out the cathode machine to clear a cathode out from between the transverse. The EE missed a step and struck their left elbow on the chain causing a laceration.

Employee was performing post-op on Peterbilt truck. Employee opened hood and let hood free fall to resting position. Safety cable failed and hood struck employee on head and shoulder and hip resulting in a lower back strain.

Employee was attempting to pull a section of belt back together with a chain puller for a splice. Because the belt was under tension the handle pulled hard, due to slack in the belt it very quickly went much easier causing the handle to loosen, when that happened it slammed down and pinched right thumb between the handle and the chain. This broke the nail and cut the thumb.

Employee obtained a steel bar used to remove slide and discovered it was stuck. Used the bar to bang on it a few times to loosen it up and attempting to lift and remove again, causing a lower back strain.

Employee was lifting a port a power from a box when EE's foot slipped causing EE to drop the port a power back into the box. When the port a power fell it struck EE's left hand resulting in a fractured finger and laceration requiring stitches.

Employee was positioning a HILTI Hammer-Drill inside the re-bar to be able to drill the next hole for re-bar when EE's left hand made contact with the trigger engaging the drill while right hand was on the bit. EE's glove became entangled in the bit causing dislocated fingers and lacerations on the right hand.

A miner operating a CAT 980 loader developed back pain over the past several months resulting in a modified duty to take a 30 minute break for every 3 hours of mobile equipment operation.

The incident was not reportable until the employee asked to see a doctor on Sept. 7. The employee was removing dust covers from the top of a railcar and struck hardhat on the dust cover holder when the employee stood up. The diagnosis was a strained neck muscle. Employee was prescribed two medications & restricted from carrying or lifting more than 5 pounds until Sept. 12.

EE stated Continuous Miner lost power so EE walked down to power center. When they arrived section electrician was trouble shooting C/M cathead. Electrician then said they were about to put the power in on C/M. In the process of putting power on the machine a cable splice grounded, causing the cable to blow up beside the injured employees face.

While removing accumulator on HP500, the accumulator fell unexpectedly. While attempting to catch the accumulator, ee's index finger became caught between the accumulator and side of the crusher mainframe. EE's finger was cut through gloves and required 7 stitches.

The injured employee was on the ground, as a rigger for the crane operator. EE and another employee were hooking up the crane cables to the crusher operator building platform. The injured employee stepped up onto the platform to grab the hook, when EE stepped through a hole in the platform. The edge of the expanded metal deck caused a laceration to EE's R/ shin.

The employee was pulling themselves up to get onto loader and felt something pull below the right shoulder blade. On September 26, the employee requested medical attention. The employee was prescribed a muscle relaxer and was not placed on restrictions.

While cutting legs free on a refeed hopper at the jaw, a piece of rust popped and went behind employee's PPE. Employee was seen by a physician and diagnosed with a right eye abrasion.

Re-installing the permanent wheel chocks to the north pad of the Code 80 fuel island. A metal shard on the pin head caught the latex glove on right middle finger. As they jerked their hand away a shard tore the skin on right middle finger. The laceration required 5 sutures to close the wound. The attendant was given a return to work with care to the injury the same day.

EE was working on removing old bolts from a flange with a bull pin and hammer. One of the hammer strikes made light contact with the bull pin. EE felt something on hand. EE began to bleed lightly on left hand. EE had sliver of metal go in to the flesh between the knuckles. Sent to clinic to have sliver removed.

Miner was working to clear an obstruction on a wash down hose. The line was pressurized causing the end of the hose to fly up striking the miner in left eye. Eye drops were prescribed.

Employee was picking up scrap metal and a jack fell and struck EE on the right side of EE's back. EE stated that a jack had come loose for an unknown reason. At that time EE was taken to the emergency where EE received 17 stitches and was released.

Heavy rains created slick environment causing loader operator's equipment to slide hitting the back wheels off a hole. This jolted EE inside the cab causing them to hit head off window causing small laceration (EE was wearing seatbelt), 9/20/18 first aid case. 10/22/18 EE went to chiropractor after medical evaluation, referred them to, indicating they had a neck sprain.

While attempting to connect an SRL to the harness, the employee lost balance and fell (same level) striking left knee on a toe board. The resulting injury was a laceration requiring 5 stitches.

An employee operating Rotopacker C, noticed that 1 of the 3 belts on the bag unwinding radimat was falling off the idle pully. The employee attempted to fix it alone by tugging on the belt with left hand, while at the same time 'jogging' the belt with the manual button using right hand to try and 'walk' the belt back onto the idle pully.

While cutting a piece of rubber tubing, employee, using a utility knife, sliced the rubber towards EE's body instead of away, the blank ran up EE's thumb opening up a one-inch cut. EE was taken immediately to the local hospital and was given aid. The attending physician sewed up the cut with 8 stitches and immediately released EE to full duty.

Employee was reaching to hang the miner cable and a rock rolled from the rib and struck employee's hand. Employee did not go to the Dr. until later on 9/21/18 and it was determined that the employee had a fractured left 5th carpal bone.

While installing additional roof bolts into a sulfur ball, the sulfur ball fell out of the roof, striking the employee's left arm and elbow. Employee received sutures to the left elbow.

Employee stated ee and another employee were attempting to load a mass jack into a scoop on the Unit 2 section. In the process of lifting the mass jack into the scoop bucket, the jack slipped causing the employee to smash finger between the jack and the scoop bucket. The employee sought treatment and received 10 stitches.

Ingestion of radiations, caustics, toxic and noxious substances

EE took an OTC pain reliever, not provided by the company and had an allergic reaction to it, which included difficulty breathing, dizziness, weakness and an itchy body. Dicalite has an AEMT II that took care of EE until the ambulance arrived. EE was given oxygen and an IV and was transported to the hospital. The Vacaville office advised Dicalite to fill out this report.

The employee was struck by a piece of roof material measuring 3 feet by 2 feet by 3 inches. The employee mentioned that employee had lost consciousness upon being struck by the roof material. MSHA was immediately notified under part 50, possible serious injury.

Struck against stationary object

Mechanic suffered a contusion to right shin that required stitches. EE was assisting in the removal of a final drive on a CAT D-11 track dozer at the time of the incident.

Struck by flying object

A piece of bearing collar broke off after being hit by a hammer and punctured IW lower lip.2

Struck by falling object

Employee was walking through a partially hung check curtain and a small piece of rock fell from the roof/rib and struck EE on the side of the face causing a laceration. 6 Stitches to close. No work missed as a result of this incident.

Struck against stationary object

Employee was removing a spreader bar from the boom of the continuous miner when a sharp piece of metal on the spreader bar cut employee's hand resulting in a laceration that required 3 sutures. No Lost or Restricted work days.

Struck by flying object

Employee was preparing to change a roller on a 345 Track hoe by removing dried mud from the roller and bracket. A dead blow hammer was used to knock off the dried mud. In doing so, the hammer came in contact with the roller and a metal shard come off the roller and punctured the mechanics stomach.

Struck against stationary object

Employee was on knees cutting plastic bands off of a new joint of miner waterline for replacement. EE lost balance causing EE's weight to shift. As EE tried to regain balance, EE cut left thumb.

Caught in, under or between a moving and a stationary object

EE was looking at a seal as the crusher door was being closed, EE rested EE's hand on the housing and pinched EE's left thumb between the door and the housing. X-rays were negative and no stitches were required the employee had to have the thumbnail removed.

Absorption of radiations, caustics, toxic and noxious substances

Employee was using the porta-john toilet when ee felt discomfort to the lower right outside calf muscle. The employee indicated that ee was bitten by a spider. There was no evidence of any type of insect in the porta-john. There were however spider webs observed in the corners.

Struck by falling object

Injured was getting ready to rock dust # 4 heading; a piece of rock fell from the roof striking ee on the head, knocking ee to the ground. EE walked from # 4 entry to # 2 entry to tell the boss about the accident. They immediately treated ee, and transported ee to the surface.

Struck by falling object

Injury occurred 09/17/18, but did not become reportable until 11/30/18 (dental care reqd). EE (injured) and another mechanic completed welding in the screenhouse. Co-worker accidentally dropped a bag of welding rods onto EE's face, causing a broken upper tooth. Dental care in late Nov was reqd.

Struck by flying object

Knocking off a piece of angle iron and it bounced back and contacted upper lip

Caught in, under or between a moving and a stationary object

Employee was installing a new haulage tram motor on the shear, as the employee moved a hydraulic hose out of the way, the motor shifted and smashed the employee's right index finger between the motor and the frame of the machine. 7 stitches were required to close the wound.
