Accident type, without injuries

Two small grass/bush fires were started by lightning near the shop and office complex at Trapper the evening of 8/31/18. There were no mining activities near the points of ignition. These fires are being documented with a 7000-1 report as they were not fully extinguished within 30 minutes of discovery and MSHA was contacted immediately.

An unintentional roof fall occurred at the Xc-31 in the #10 Entry of the #2 unit. The fall was approximately 40' x 18' x 12'. The fall was barricaded off and a site specific plan to rehabilitate the immediate area was put in place.

While making weekly airway exam a roof fall in the cross cut between #6 entry & #7 entry one break outby spad 510 was found to have fallen. Fall was reported as being from the right corner of the #6 entry through the intersection of the #7 entry approximately 6 ft from the coal rib and approximately 6-8 ft high.

As a lightning storm passed over the mine a bolt of lightning struck our incoming power. Power was lost. It was found that a bank of transformers had been struck and one was burning. The fire was extinguished but rekindled later. At that time both Hot Lines (MSHA and State) were notified .While talking with MSHA I was notified that the fire had been extinguished.

An unplanned roof fall occurred out by survey spad #583 approximately 20' long and at least 6' high in the intersection of the number 2 entry. Adverse geological conditions were visible.

An unplanned roof fall occurred in the Cedar Grove Mains Return, at #29 X-cut, #8 entry. The fall is approximately 20 feet long x 20 feet wide x 5 to 6 feet high.

#3 Hoist Galena Mine - Pulling muck on #1 side, power dropped out, night shift crew on surface. Avista power line coming on site insulator broke and dropped power out back to Osburn Sub-station.

A non-injury roof fall occurred in the number three entry on the pillar line one crosscut inby survey station number 9624. The continuous miner was caught during pillar recovery on the 001-0 MMU. The pillar and roof control plans were changed to allow recovery of the continuous miner and normal mining operations have resumed.

An unintentional roof fall occurred in 3 North Mains in #10 entry from 9 wall to 11 wall. The fall measured approximately 12'-15' high by 16' wide and 450' long. There were no injuries as a result of this incident. The event occurred due to humidity in the area and a lateral shift which caused the 8' bolts to fail at there couplers.

A roof fall occurred above the anchorage point on the old mainline near the entrance to 4 south. The fall was approximately 12'long, 6'high and 16' wide. The cause of the fall was due to weathering and additional support will be installed in areas that show weathering.

A fall of the roof occurred in the 2nd SE Mains (3rd54 Supply Road), #5 entry, crosscut #36, intersection. The fall measured approximately 20ft. in length, 19ft. in length, & 7-15ft. in height. A fall rehabilitation letter has been submitted to the local MSHA office.

A non-injury roof fall was found on the secondary escape way on the Spraddle Branch Mains at spad #1236.

An unintentional roof fall occurred in the 4 West area of the mine. The original fall was located in the # 4 entry, Later in the shift it fell in the #2 entry, which is the belt line. During the investigation is was determined that a slip was running through that area that caused the roof to fall above the anchorage.

During inspection, a cable connection was found to be installed not according to manufacturer's recommendations on the Emergency Hoist. All EE's were withdrawn from the face area to outby the feeder dumping point while adjustments were made. No persons were injured or killed during this event and the alternate escapeway was still available. The repairs were completed in two hours.

An unintentional roof fall occurred in the G1 Head Gate section at 49 wall in the #3 entry and into the 3 to 2 crosscut. The fall measured approximately 18' high x 31' long in the #3 entry x 37' long into the 3 to 2 crosscut. The immediate roof consisted of laminated shale material plus adverse geological conditions contributed to the failure.

Approx. 12:40 pm on 8-23-18 the Mine Foreman discovered a prior roof fall at 258 1/2 block in the 6 North Left Return had continued to deteriorate. The fall is now approx. 16 ft.W x 20-25 ft. L & 12-14 ft. high. Initial fall was approx. 5-7 ft. high. No injuries, equipment damage, or impaired ventilation as a result of the fall.

The safety switch on top of the elevator tripped, causing a fault. This caused the elevator to stop while in motion.

An unplanned roof fall was discovered at 3-04 intersection on the haulage entry. The fall is approximately 20' long, 10' high and 16' long. The fall has been cleaned and re-supported.

A unplanned roof fall was discovered on the South Bleeder Track, at #22 X-cut. The approximate size of the fall was 18 to 20 feet long x 18 feet wide x 4-1/2 - 5 feet high.

Roof fall in the left return area of 1 section between 29 and 30 block. Also roof fall in the right return,#9 entry between 34 and 35 block. The cause of the fall was adverse geological conditions.

An employee became stuck in the Primary Crusher elevator. Electricians were called to reset it. At the same time there was a power bump and red alert. At 6:26 p.m. the employee was released from the elevator. The elevator was locked out until an inspection and repairs are made by the elevator company.

A roof fall, in an examination line of travel, was found during a pre-shift examination in our 6W Mains. The fall was located at X-Cut #5 in Entry #8. The fall did not impede passage of miners, affect ventilation or cause personal injury to miners.

An unplanned roof fall occurred on the 8B Belt at X-cut #2 inby spad #7924. The fall was about 70 foot long, 19 foot wide and 8 foot high. The fall will be clean up and rehabilitation to the cleanup plan.

A fall of the roof occurred on the 1st NE Mains (1st54 Supply Road), #5 entry, crosscut #36, intersection. The fall was approximately 20ft. length, 20ft. width, & 7-15ft in thickness. A fall loading plan has been submitted to the local MSHA District 10 Office.

A hot spot was found on the raw coal stock pile. Citation number 9130369 was issued on 8/20/18 when it was determined that the alleged fire was not reported. There was no visible flame witnessed, and the hot spot area was rendered harmless.

Employee was in the Step 3 elevator when it stopped unexpectedly, trapping person in the elevator. Employee used the phone to call for help. MEI Elevator was called. MSHA was notified of the incident in a timely manner. MSHA returned a call asking to be notified when person was out of elevator. Notified MSHA that person was released at 12:12pm. MSHA returned second call @ 12:32 pm

On 8/15/2018, ignition of anfo in 229/21 area tore down utilities, damaged glass on blasting rig, and sent 3 miners to the ER to be evaluated. The miners were released to return to work without restrictions.

Flash flooding occurred and water from Tunkhannock creek went over the bank and increased the water level in the pit by 6'. This occurred overnight when no one was in the mine. No damage occurred.

An unplanned roof fall was discovered at 7-33 in the Conner's Run Return. The fall is approximately 10' high, 20' long and 16' wide.

Communication fault on drive system of production hoist. Troubleshooting identified the problem as a defective communication cable. The cable was removed and replaced the hoist was tested and put back into service.

A roof fall occurred in the 9 seam panel 6 between xcuts 48 - 49 in the #6 entry. The roof fall measured approximately 50'L x 19'W x 10'T. The fall will not be cleaned up. The roof was supported with 5' fully grouted roofbolts and 8' megabolts used as supplemental. Date of development 7/2018.

The roof fall was located on B-2 Mains at crosscut #41 in the #2 entry. The fall measured 20' by 20' and was 10' in thickness. The area was previously supported by with 5' Hi Tech bolts and 12' cable bolts. The roadway and lifeline was re-routed and the fall will be cleaned up. No persons or ventilation was affected

On 8-13-18 a roof fall occurred on the old mainline at survey station 345+00. The fall was 85 feet long 16 feet wide and 7-8 feet high. The fall occurred due to weathering and additional support will be installed in conjunction with cable bolts. No other part 50 criteria applies.

A roof fall was discovered on this date. The fall was located in the #1 entry of A-Mains between crosscuts 27 and 29. The fall was of various heights. The area was previously supported with 5' Hi Tech bolts. No persons or ventilation was affected and the fall is being cleaned up.

During a thunder storm, an electrical phase burned in two causing a power outage. The power line supplied power to the bath house and the elevator. An employee was on the elevator at the time of the power outage. The employee was stranded on the elevator for approximately 65 minutes.

A roof fall occurred in the 9 seam panel 6 between xcuts 46-47 entry 7. The roof fall measured 50'L x 19'W x 10'T. The roof fall will not be cleaned up. The roof was supported with 5' fully grouted roofbolts and 8' megabolts as supplemental. Date of development 7/2018.

A roof fall was located on shift exam @ 4:25AM. Roof fall occurred one break inby SPAD 29, Entry #5. The roof fall was approximately 20'W, 25'L and 3'W.

All plant power was lost due to an area wide blackout impacting the entire west side of Cleveland. Power was restored by CPP at approx 2 am.

During the daily service check of the elevator the computer program faulted out at 5:25am and could not be reset.

Unplanned roof fall discovered reportable in the tight side intersection of 12 block on 4 belt. Area previously cavitated and brow (approx. 12' long, 6' wide and 6' high) fell out, three roof bolts were found in the rubble. Cavity and rubble had a heavy layer of oxidized sulfur.

A roof fall was discovered on this date while conducting other work in this area. The fall measured 20' by 20' and was 6-8' in thickness. The fall was located in the #1 entry of the 3-North-2 Mains at crosscut #11. The area was originally supported with 5' Hi-Tech bolts. The area was dangered off. No persons or ventilation was affected.

Production hoist brake line failed and needed to be replaced. *After repair was made to brake line hoisting system could not clear a brake position fault. This was not a secondary issue it was part of the previously reported instance. No one was underground at the time this issue was identified during test runs.*

At 9:42 am the hoist at Camp Creek portal went down due to a control stop fault that showed up on the computer screen and would not reset. The company got another computer and reset the fault at 10:10 am.

A roof fall occurred in the 9 seam in the 1st Northeast Mains at crosscut 5 entry 4. The roof fall measured approximately 16'L X 18'W X 8'T. The roof fall will not be cleaned up. The roof was supported with 6' fully grouted roof bolts and date of development was 7/2018.

A grass fire started in the old B-Area, caused when a hay baler caught fire. The fire was contained with 15-20 min, but the hay bales within the burnt area were still on fire.

An unintentional roof fall occurred in the Cedar Grove Mains, at #11 XC, #5 belt entry in the intersection. The fall is approximately 18 feet wide, 20 feet long, and 6 feet high.

The American Eagle Mine experienced an unplanned roof fall above the anchorage zone in between 76 and 77 breaks on the old 2-South mains in the #7 entry. This is not an escape way and does not alter ventilation. The dimensions are approximately 50' long 18' wide and approx. 8' thick. No injuries occurred during fall.

On the evening shift of 08/17/2018, a roof fall above anchorage occurred at the outby end of the Mobile Roof Support, the fall was in the #6 entry, 80' inby spad #3640. It measured 6' thick- 20' wide -23' long.

While rehab bolting in the bottom of the B-Mains two cribs where required to be removed to bolt a brow, after the roof bolting resumed the top started to settle on the ATRS of the roof bolter.

The crabapple hoist was taken out of service for an extended period of time. The hoist rope was found to be near the minimum tolerance and the decision was made to remove this area of the rope.

An unplanned roof fall occurred inby the section loading point at the 15 bk intersection in the #4 entry on D-Mains (MMU 001-0) section. There were no injuries, equipment damage or disruption to the ventilation system. This occurrence was immediately reported.

A miner was switching equipment when ee heard a small air leak, during investigation for the air leak ee heard something falling to the ground, upon arrival to the 812 location ee discovered a large portion of the right hand rib, spring line and back had fell to ground.

Roof Fall 5th South Panel entry 3 xc3. Maintenance EE found the fall around 1249am and reported to Management and they reported to the MSHA Immediate Accident number then went to get roof fall secured with timbers and dangered off and started investigation of fall. Fall was 18'x25'x10' hi bolted with 8' tr bolts, cable bolts, screened. Fall is secured.

A portion of the highwall over the number #1 fan portal entry separated from the highwall and rolled to the ground. There were no miners exposed. There were no injuries. The MSHA hotline number was called.

A roof fall occurred on the Main West at 4+00 in Entry #6. The roof fall was discovered by the pre-shift examiner.

Kiln 5 was shedding ash ring, the cooler pit could not cool the lime/ash down enough and the hot material caused the conveyor belt to ignite. Fire Department was called and extinguished the fire rapidly. The conveyor belt and some conveyor rollers were damaged.

An unexpected roof fall occurred in the track entry in 6 South section at 59 X-cut. This incident was caused by adverse geological conditions in the form of a slip that was identified in this area after the fall was located. The fall measured approx. 30 ft long by 20 ft wide by 10 ft high. No equipment damage of injuries as a result of this event.

Roof fall occurred in TG #2 right side return. Crosscut 5 between entry #2 and #3 at the mouth of HG #6 (overcast line). An action plan was submitted and approved by MSHA.

Victim used an aerial lift positioned under the #3 tunnel belt to access the take-up pulley assemble components. As the victim was attempting to manually clean the north side of the west bend pulley, ee became entangled between the rotating pulley and the moving conveyor belt, resulting in a fatal injuries.

Employee was bolting when they reached out to pick up bolt, a piece of rock measuring 32""x12""X4"" fell striking them in the right hand amputating the tip of the pointer finger.

Caught in, under or between running or meshing objects

A 23 year employee-electrician, was replacing the reverse air fan on the Kiln 2 main baghouse. The fan had been locked out but the motor drive belt to the fan wheel was still rotating. EE attempted to stop it by grabbing the belt. Their right hand pulled by a drive belt into a sheave pinching their pointer finger.

IE was helping with changing v-belts. The belts were put back on but 1 belt was twisted, so another EE was using a pipe wrench on the shaft and it slipped off causing the belt to roll back pinching IE's thumb.

Installing J-Seal when worker caught glove in the auger which cut off tip of index finger up to the first joint on the left hand.

Struck by falling object

While removing the bolts from the counterweight of a Hitachi ZX850H, the counterweight fell backwards onto employee's lower right leg, severing it above the ankle.

Employee was standing in the intersection, when a piece of draw rock fell striking their left foot. The rock measured 24""x 25"".

Employee was in break room, turned to pick up drink can, turned twisted knee and felt a ""pop"". Complained of difficulty bearing weight on R leg. Evaluated in medical went home. Returned next day and continued to work regular schedule, not missing any work. Was seen by MD 09/10/2018 and had MRI 09/24/2018. Returned to work from MD visit wearing brace. Last day of work 09/30/2018.

An employee was struck by 4"" waterline that came out of scoop bucket causing a fracture to right lower leg. Surgery was required to repair the fracture.

Employee was knelt down speaking with the preceding shift crew members on the 9O CM Unit, as the employee attempted to stand up from a kneeling position foot slipped and the employee twisted right knee.

Contact with hot objects or substances

Employees were in process of removing a pump from a mine shaft. When one employee started to cut a drive shaft coupling with a torch, a gas ignition occurred causing burns to another employee.

Employee was descending ladder to bottom floor, once on the bottom floor ee turned to adjust a valve and stepped on an uneven surface that was under water. Employee started missing 09/28/18.

Caught in, under or between running or meshing objects

Employee stated that they noticed a rock hung between the bottom belt and the belt structure, they began striking the rock with a two foot bolt wrench, while striking the rock, the roller grabbed the wrench jerking it out of their hand causing their hand to slam off the belt structure.

Struck against a moving object

Individual operating scraper, traveling empty along mine floor, moved off track to allow truck traffic. Hit rough area causing EE to hit head against right and left side of cab, leaving EE unconscious. EE believes EE began to can down before hitting head, which stopped the scraper shortly after EE lost consciousness.

Struck by falling object

Employee bolting SR punch through, a section of rock rib above the coal rolled off striking employee. Rock was 12' L, 2' W and 10"" thick. Legs were trapped under one end of rock. Employee complained of leg pain and then back pain as employee was being removed from mine. Employee has fractures to right ankle, both sides of pelvis, spine and ribs.

Contact with hot objects or substances

Employee tried to start a fire in a furnace attached to a cyclone. EE decided to use gasoline to start the fire. EE opened the door because ee thought it had not ignited. It allowed air to rush in which caused the gas to ignite quickly. It burned right arm and slightly right cheek.

Caught in, under or between a moving and a stationary object

Employee was holding a cable on the flow gate on a feeder when the flow gate slipped and pulled EE's finger through the pully with the cable injuring the finger.

Struck against a moving object

Miner was operating a wheel loader moving material in the pit for several hours. Upon exiting the machine the miner said back was stiff and sore. The condition worsened over the holiday weekend and the miner went to the walk in clinic on September 4th. Doctor advised they should be excused from work for September 4th. Employee did not report injury until September 4th.

Struck by falling object

Employee was roof bolting on the return side of the miner in the 5A section. While drilling a bolt hole a rock approx. 4' x 3' x 1"" came out of the roof area and struck EE's right lower leg causing a laceration, requiring sutures.

Caught in, under or between a moving and a stationary object

Employee left hand middle finger was smashed in a pinch point between belt cover door while opening the door causing a laceration requiring six (6) or more sutures.

Over-exertion in pulling or pushing objects

Injured employee was roof bolting on the 1W Tailgate Section. As ee was installing an 8 foot roof bolt, using left hand to push the bolt up into the bolt hole ee felt pain in left shoulder. EE continued to work until 10-5-18 when ee began missing work due to this injury. Employee has training & was wearing proper PPE. Did not violate any rules or regulations.

Struck against a moving object

In a dip on old #6 Belt Track, the employee was a passenger in the first of two man buses. As the first man bus went down the dip and was topping over, it flashed its lights for the second bus to proceed, when the first bus stalled out and rolled back down causing the second bus to hit it. This employee began missing work on 06/04/2019.

The driver was driving on a raised dirt road when the berm of the road broke and the mixer truck went over the edge. The truck rolled onto its side. The driver was taken to the hospital and received 12 stitches on forehead.

Employee was driving to work in EE's personal pickup truck on the haul road, when EE lost control of EE's truck, causing EE to ride up on the berm and roll EE's truck one time.

Struck by falling object

Employee was backing the continuous miner when the cutter head of the miner contacted the coal rib on the right side causing the corner to become loose. Employee was unaware of this and went to the cutter head to move the miner cable over. The corner of the rib came out and struck employee's right ankle resulting in a fracture of the tibia and fibula.

While installing a BFR door to the longwall stage loader a water jack became dislodged striking an employee across the lower legs. The water jack was used to keep tension to the BFR door in order to connect the ram jack pin to the door.

Employee was carrying bolts to the longwall face for the pan line bolter. When they set the bolts down onto the tray, they sliced their forearm on a bolt plate that was laying in the tray, causing a laceration.

Struck by flying object

Co-workers just completed drilling holes in angle iron. Employee blew away metal shavings with their breath. Shaving went in their eye. Employee did have safety glasses on.

Employee was servicing a front end loader and was walking away from loader tripped and fell to the ground catching themselves with right hand to break fall.

Struck against stationary object

An employee was performing a pre-shift inspection when EE stepped on a rock and rolled EE's ankle causing EE to fall to the ground, landing on EE's knee.

Over-exertion in pulling or pushing objects

The employee stated EE was using a bar to push a rock out of 6M rock box and EE felt pain in EE's neck and left shoulder.

EE was adjusting a conveyor belt with an adjustment bar when they felt a painful strain in the lower back. EE worked the rest of that day, and the next day. Back pain worsened. EE has since attended the doctor and has been prescribed muscle relaxation medication and physical therapy. EE plans to return to work on 09-04-2018 on light duty.

Employee was walking along stage loader when ee stepped in a hole and twisted right knee. The employee continued to work until 9/24/2018 when ee was taken off work.

Fall to lower level, (Not Elsewhere Classified)

Employee climbed over a barricade and travelled in a non-designated area. Employee was stepping over a gap between a retaining wall and a primary jaw crusher and did not step far enough. The height from the top of the wall to the floor is approx. 11 ft. The employee stated bifocal sunglasses made employee misjudge the distance employee needed to step.

Employee was cleaning the window on the haul truck when they fell injuring their right foot.

injured employee was stacking (6x6x36) crib blocks for section tail piece and mashed EE's left pinky finger had to have 6 stitches put in EE's pinky finger.

Injured Person stated while changing out a bottom roller IP hung one side and went to pull up to hook other side and the side IP first hooked came off causing the bottom roller to strike IP in right side. There was no witness and IP didn't tell nobody till next day.

Waiting to see if surgery is required for the orbital bone. Doctors expect a full recovery.

EE was shoveling out material along the sand conveyer. EE stood up and when EE turned to exit the area EE had a pain in lower back. EE called to get some help to walk down the cat walk and was sent to providence health to get checked out for the pain.

Employee was driving an empty shuttle car toward the miner when employee hit a hole or dip in the roadway causing employee to strain lower back, neck, and right foot.

Employee was driving a rock truck when employee got a call for it to be serviced. Employee then parked it at the truck garage. While entering the replacement truck employee slipped and fell on shoulder/back. Employee attempted to maintain 3 point contact and wasn't distracted by talking.

Installing a 6 foot bolt with a strap. The bolt began to bend over, the individual stopped to straighten the bolt by striking it with a wrench. The strap slid down the bolt while EE was swinging the wrench and EE struck hand on the strap, lacerating EE's fingers.

Employee was using a one inch impact gun to remove nuts holding the bucket segments on a loader. The bolt spun and cut the employee's palm on their left hand.

Attempting to remove a pin that was holding a gate up under the crossover screw. While trying to lift gate arm up employee strained the lower groin area. Occupational Medicine review indicated a hernia. Surgery to repair occurred on October 9, 2018. Employee worked their position until then without restrictions.

Miner was lifting discharge line that was suspended to couple two joints of line together. Miner experienced discomfort in right rib cage area.

Removing a bad bushing from a D10 back bar. Employee hit the bushing to shift it over so that a retaining ring could be removed. Grease and metal slivers came back and hit the employee in the eye.

A fully trained miner was mixing B-Bond when the dust from the bucket came into contact with EE's eyes. The injury was not the result of any compliance issues nor were any protective guards involved.

While trying to recover the mule from the mud, employee was hit in the head by a hook that broke off the mast of the mule.

Employee was attempting to hook a pump to the back of a truck. The pump rocked back and forth and the tongue fell, striking the employee's left foot. EE continued to work until EE went to the Dr. on 8-11-18. EE's first day missed was 8-13-18.

Employee was operating the tailgate drum of the shearer in the F28 LW section, mining towards the tail when a rock came over the handrail out of the panline and struck the employee on the inside of the right leg just above the ankle. Incident resulted in fractures to the tibia and fibula in right leg that required surgery.

The last 2 sections of the dryer bed were being removed when they became wedged in the framework during the hoisting process. The employee entered the dryer using the ladder on the inside wall to try and use foot to free the piece. As EE was using foot to try and free the plate it moved and ee struck ee, ee lost balance and fell to the dryer floor.

At 9:00 am on 7/9/18. Employees knee was bothering EE while crawling. Went to the doctor on 8/2/18 MRI showed a medial meniscus tear on right knee. Employee worked until 8/23/18 is now getting surgery today.

On 11/19/18 an MRI determined a torn meniscus and EE was released to restricted duty.

The stab jack pin broke on the operator side drill station canopy. The roof bolter operator set the jack on the ground in order to begin bolting. As a result of the broken pin, the canopy lowered when bolter operator pressurized the stab jack. The canopy lowered on operator's head resulting in a strained neck and shoulder.

Employee was operating a 2-person golf cart. EE did not notice a low spot in the roof and pushed head between the roof and golf cart. Employee continued to work until 02/08/2019 when surgery was performed to repair vertebrae.

Received three stitches on finger nail from ER and returned to full duty at work the next day. Went to a specialist a week later on 08/27/2018 and they said that the tip of finger is broken. EE is on light duty work now.

IE was removing a tension bolt on a screen to change screen. Utilizing an impact wrench to remove the bolt ee was guiding the washer/bolt with left hand. EE activated the impact wrench and ee fingers were caught/pinched/grabbed and caused ee to lose skin/tissue on palm side of middle finger.

Employee was standing in the warehouse when EE reached into the right pant pocket with right hand to grab the radio when EE had a low back spasm causing EE to go down to one knee.

Miner was walking to the parking lot at the end of EE's shift. EE removed EE's half face respirator while enroute and took EE's eyes off the ground EE was walking on. As a consequence, EE tripped on the pavement, and fell landing on EE's right shoulder.

Lifting low belt structure stand and swinging it with both arms to place it underneath the conveyor belt, the stand hit a ledge or mud and suddenly stopped movement. This caused a severe ""pop"" sound from employee's right inside elbow and lower bicep area.

Employee set an extension ladder up, upside down, with the hard plastic on the ground. Employee began to climb the ladder, got 7' up, and the ladder fell out from under ee.

The employee went to the roof with other workers to set a piece of tin roofing in place. The roofing is notched out so that it fits in a certain spot on the roof. The employee grabbed the tin in the middle section where it was previously cut by a saw and had bur marks. EE's glove caught the burs and cut through it cutting four fingers on the right hand.

The dust collector pipe fell approximately 4 inches hitting EE on EE's hardhat and pinning EE's left forearm between the wall, existing conduit with 24 inch dust collector pipe.

Was unhooking a chain on a conveyor. Thought the conveyor was on the ground but was hung up on something. The conveyor released from whatever was keeping it up and dropped on right foot.

Employee was weighing beads in the lab and dropped one. When they bent down to pick it up they hit the back of their head on the corner of a lab table after picking up the bead and coming back up. Employee did not report the incident until they arrived at the hospital on 8/11/2018. It was then determined that they had a mild concussion.

The maintenance crew began working on the elevator and when the backstop and tensioner were removed the buckets rolled slightly and a come-a-long smashed the injured employees hand against the structure. Employee figured they could use the 1/4 bolt on the one side of the shaft for leverage to remove the shaft from the base. As they picked up with the loader to remove the pressure it smashed employee's finger between the bolt head and stand base.

Struck against a moving object

Employee was driving a mantrip and they hit their face on the mantrip when it struck a rail causing injury.

Struck against stationary object

The employee picked up two bottom roof bolts and was setting them on the tray of the miner, when EE struck left hand on the corner of a rib strap lacerating the left knuckle. The employee was wearing all appropriate PPE and was not violating any laws or company policies.

Struck against stationary object

Employee was straightening a cover for a hydraulic tank and tipped over a piece of wear plate that was on the work bench and it struck left ring finger. Notification was received on 8/23 that employee was diagnosed with a finger fracture.
