During the weekly examination, a reportable roof fall was found in F-Seam, Southwest Main Intakes, entry 3, crosscut 29. The fall was approximately 18' wide, 18' long, and 7' high.

An unplanned roof fall occurred 5800' from active face in entry #2 at spad 12756. The fall measured 15' in length, 16' in width and 3' high. Roof was supported by 48"" resin bolts and fall was rehabilitated.
Hoist person was painting and one micro switch was knocked out of adjustment that would not allow the hoist to start. The electrican had to re-adjust the switch so the hoist could run.
Knight Hawk Coal LLC, Royal Falcon Mine found a roof fall in the #1 entry of the Main West at the +430 cross cut in the return. The fall did not obstruct any escapeways, air courses, and was outby the tailpiece of the section. The fall was called in to MSHA and we have done what our district asked of us. No employees or equipment were involved in the fall.
An alleged ignition occurred near number 70 shield on the 14A longwall face while mining towards the head gate.
A roof fall was discovered at North G 24x-cut#6 hdg. intersection only, 1 x-cut in-by the feeder. Fall was 10 feet long, 20 feet wide and 12 feet high. No men or equipment were injured or trapped. no ventilation or transportation were affected. The area was bolted with 6 foot super twist bolts and 12 foot post tensioned cable bolts.
IGNITION: The Miner was cutting a large pot that had fallen out of the mine roof in the middle of the No. 2 Entry Face, when the ignition occurred. The flame was on the operator side of the Miner Head. The flame was put out with one fire extinguisher.
Starting in intersection in last open xc outby 70' to 90', an unplanned roof fall occurred in #1 entry on #1 unit starting at 23+80 to 24+50. There is a slip running through this area. The area has been timbered out and cribbed off and flagged off to keep anyone from entering. Fall was 14' to 18' thick and 8' high.
EE was operating a 2004 Volvo 220 front end loader clearing snow off the haul road when he heard a loud pop. EE noted smoke & flames were coming from under the engine cowling. EE dropped bucket, set brake, turned machine off & exited the vehicle. After briefly trying to extinguish flames with hand held extinguisher, the fire dept was called. They extinguished fire. No injuries.
A roof fall occurred at spad #4+80 in the no. 3 entry in the first main west supply road....width 20 ft x length 30 ft x high 12 ft.
A roof fall occurred #2 section return 2000' outby section inby spad #4926. Fall measured 20' long, 18' wide & 6' thick. No injuries, no passage of men affected.
Unintentional roof fall occurred in the intersection in #2 entry #161 crosscut. Fall is 30'L x 18'W x 10'H.
Roof fall in #2 entry of the mine mouth. The fall measured 4' high, 4'wide, and 15' long. The fall opened a gap between the highwall and fan housing. No equipment was damaged or injuries occured b/c of the fall.
An unintentional roof fall has occurred at XC 13 of the Main West Return. The fall was discovered during the weekly examination of the return. The fall is approximately 20 feet in length and 19.5 feet in width and 7.5 feet high.
A roof fall occurred on 1st right panel #2 entry 900' outby the working section, spad #990 fall measured 100' long, 18' wide and 5' thick. No injuries or passage of men affected.
The #8 elevator was inoperable for a period of time exceeding 30 minutes due to the gate switch being out of adjustment. The gate switch was adjusted and the elevator was placed back into service 12:10 AM. All agencies were notified according to law and the escapeways were re-routed. No individuals were on the elevator at the time.
Unintentional roof fall was discovered during a weekly exam prior to beginning the work week after vacation. Fall was located in #4 entry between crosscuts #9 & #10 in HG #2 set up rooms. Fall measured 40' L x 8' W x 8'H.

#3 Hoist Kicked out on ""Air Brake Pressure"" fault. One of the four air brakes was losing pressure so the brake needed to be replaced. There were still two operable hoists at the time of the incident (#3 Hoist is the production hoist and secondary escape hoist).
Unintentional roof fall was discovered during a weekly exam prior to beginning the work week after vacation. Fall was located in #2 entry #2 intersection in HG #2 set up rooms. Fall measured 50' L x 18' W x 9'H
A roof fall was discovered at 3 main 14x-cut #6 hdg. 4 way intersection. Fall was 20 feet long 20 feet wide and 14 feet high. No men or equipment were injured or trapped, no ventilation or transportation were affected. Fall was 5,000 feet out-by the section. The area was bolted with 6 foot fully grouted resin bolts and 12 foot cable bolts on 8x8 foot pattern.
3M front end loader operator was removing snow while plant was shut down. A Per Mar Security guard pulled up in a truck behind loader. Loader operator backed into truck. No injuries.
The Tyro Creek Service Elevator was out of service from 9:00 am to 1:45 pm due to power outage.
A shovel operator discovered an emulsion column in a dig face. He backed the shovel out of the face and called his supervisor Re: the powder column. He was cleaning the dig face floor with the shovel bucket when a booster detonated. Minor damage occurred to the shovel. MSHA was notified 9 minutes after determining the incident was reportable
There was a roof fall in the 2nd Northwest Parallel. The fall is 19 feet long, 20 feet wide and 8 feet high.
A roof fall occured in the intake escapeway in the #3 entry, approximately 3 breaks inby blowing fan. The fall measured approximately 70' long, 6' high and 16' wide. The fall will be cleaned up. The fall effected passage of men and ventilation.
A roof fall was discovered by the mine examiner while traveling the airway of 6 right south mains # 6 cross cut inby the # 4 cut through. The estimated size of the roof fall is 6 ft high, 18 ft wide and 140 ft long.
A roof fall occurred on # 4 unit, crosscut 14, # 9 entry. The fall measures approximately 7 feet long, 17 feet wide and 3 feet thick. The area is supported with 5 foot torque tension rebar bolts.
A roof fall was discovered during the mine exam of the north east #5 seal, the estimate size of the roof fall is 6' high, 18' wide, 20' long. No injuries were reported as a result of the roof fall.
The bracket that holds the guide for the door locking mechanic broke when it contacted some ice.
Roof fall was discovered 100 ft outby spad M407 in a crosscut in a return airway, #2 mains, 11,600 ft from the face, area was developed in 1992.
At 11:10 am on 12/23/08 smoke in coal pile was reported. Investigation discovered smoke coming out of the top of the coke pile. They set up the coal/coke reclaimer over the smoldering material and scraped it off. The operation was successful and the fire was out by 12:00 p.m. Since it took longer than 30 minutes, the CR Operator reported incident to MSHA at 11:40 am.
A roof fall was found in 9 mains-#5 Butt-#3 Entry approx 40' inby station #5190. The fall was approx 6' high, 19' wide and 40' long. The area was bolted with 6' fully grouted torque tensioned bolts. The area is dangered off. The fall does not impede ventilation or travel.
X-cut 2 - 1 at 7 brk s m section #4 panel fall is approx 7' - 9' high, 17' wide & approx 45' long. Cover 390' area bolted with 6' resin bolts & hole boards. A slip at 8' was visible after fall occurred. X-cut will not be cleaned up. Called State & Federal hotlines.
South 4-7 block, #5 entry, Spad #9479 MMU 006-0 17 ft wide x50 ft long x 6ft high. Support: 6ft 3/4 inch resin bolts, pans, brow straps, acs jacks and bars fall dangeged off, additional roof support installed around fall and clean up plan posted.
An unmapped mine was cut into while mining was being performed on the 001-0 MMU. A small amount of water was released into the active section. No Methane or low Oxygen was released from the old works and no hazards were noted. All Persons were withdrawn and production was immediately stopped.
An unplanned roof fall occurred on the 002-0 MMU in the #8 entry one crosscut inby Spad # 960. The fall occurred in an intersection. The fall measured approximately 40'W x 25'L x 8'H.
A roof fall occurred in the track entry on the one south track area at x-cut #10. The area was bolted with 5 ft bolts and cribs on the side. The fall will be cleaned up and the area will be rebolted and cribed. The fall was about 20 ft long, 19 ft wide and 5 to 8 foot high.
Koepe man hoist was malfunctioning. Investigation revealed the ""resolver"" on lilly controller shaft was loose. Resolver was tightened and controller put back in ""sync"". No injuries; no other problems. Hoist ran on manual until problem solved.
A roof fall occurred in the 2 South 2, #6 entry, from 17 to 18 crosscut. The fall measures approximately 20 feet long, 5 feet wide, and 3 feet thick. The area is supported with 5 foot fully grouted bolts and 10 foot point anchor bolts.
A rock fall occured in 1st Northeast main in #6 entry at spad 38 + 20. Fall measured appr. 50 ft by 19 ft by 6 foot high. Intake was blocked going to #1 unit. This area is 10,500 ft from active #1 unit.
Hoist failed to operate and electricians were called to investigate. 'Park brake' coupling had failed setting the brake. All personnel were withdrawn. MSHA was notified. Coupling was repaired and hoist placed back in service 12/23/08.
A roof fall occurred in the #6 entry at spad 19+30 to spad 20+10 on the 002MMU in the 7th sub-panel of the 2G Panel. This fall was due to a hidden slip in the area. The height ranged from 4' to 6' high, the width was 18.5', and the length was 80'. The Stamler Ram Car, model PH10 co. no. RC302 sn. 1135, had rock on the back end of the batteries.
Koepe man hoist malfunctioned. Lilly controller was out of ""sync"" due to bottom limit switch inoperable. Switch was replaced and hoist checked; back in service at 3:00am. Mine evacuated; no injuries; no other problems.
An unintentional roof fall occurred in the #4 entry at spad #9491. The fall was estimated to be 4 ft high by 18 ft wide by 60 ft long. This area was bolted with 4 ft resin bolts with 10 ft cable bolts in the intersection. No injuries, track & belt was caught, ventilation was not interupted. The fall will be cleaned up.
An unplanned roof fall at the anchorage zone where 6 ft. bolts are in use. The fall is approximately 20 ft. wide by 21 ft. long by 6 ft. high in 22 Right, Crosscut 115+14, adjacent to #1 Entry. No apparent contributing factors were identified. Cracks in the adjacent entry indicate floor heaving but the roof appears to be adequately supported with rock props and timber.
Coal that was spilled while loading a train was piled at the end of the load out tunnel. The pile of cola developed a hot spot and caught a near by grass hill on fire. The fire was put out using a water wagon and the local fire department,, In all the fire burned less than an 1/2 arce and took about 45min to put out.
Fire in an unoccupied structure: The fire is still under investigation, but it is believed to have been started by repairs associated with welding activities.
Cross Cut fell out on 1st left panel in the return, Spad # 215. We set jacks around the fall.
Power was lost to shaft and hoisting equipment due to an area wide power outage. The employee's were gathered in one area until power was restored around 12:00 PM.

A fall of the roof occurred on #2 section #1 entry approx. 70' inby spad #3178, 320' outby section. Fall measured 30' long, 20' wide, 5' thick, no injuries, no passage of men affected.
The above mentioned mine has experienced a non-injury, unintentional roof fall outby the active workings of the mine. The fall occurred in #5 entry off 1st right mains, old works. Fall was 30 feet long, 20 feet wide and app. 6 feet thick, shale. 48 Inch fully grouted rods were initially used. area dangered off and cribs installed.
Elevator down. No accidents or injuries. Went down at 0330, repaired at 1730.
The above mentioned mine has experienced a non-injury, unintentional roof fall on the #5 conveyor belt at x-cut #10. The fall strata was shale, app. 30 feet in width, app. 23 feet in width and 4 feet thick. 48 Inch fully grouted rods and 10 foot cable bolts was the initial support.
A roof fall was discovered by airway examiner on C1 panel in old worked out area. The fall was at #3 left entry spad # 2227 in 4 way intersection. Area has been timbered off. The fall was 25' wide X 40' long X 6' high. It was found at 11:15 pm on 12/18/08.
Elevator at Livingston Portal stopped while ascending entrapping an individual from 8:18 PM until 9:05 PM.
On the weekly examination of the bleeder system a roof fall was discovered in the number 1 entry between spad 227 and 244. The fall was approximately 20 feet long x 5 feet high x 160 feet long. The fall did not affect miners, ventilation nor equipment.
The elevator went down at 8:03AM with a burnt up coil and relay in the electrical system. Production was stopped, Ventilated and power pulled. People were pulled back outby the loading point.
Knight Hawk Coal LLC, Royal Falcon Mine has experienced an unintentional roof fall. The fall happened in the first set of return rooms of the 1st east panel at the +288 cross cut in the #3 entry. No persons or equipment were damaged. MSHA was contacted at 2:19. PM. The area fell at 2:15 p.m. and is now dangered off and cribed off.
Due to a power failure on the property, the secondary hoist was out of commission for more than 30 minutes. The main shaft and adit were still usable. Full power was restored after 4 PM.
Weekly examiner found a roof fall at 8th South right return #6 entry at #6 x-cut. There had been a previous roof fall in the x-cut 6-7 and the intersection fell in approx. 20' x 15' x 7-8 feet high. The area was dangered off and timber set at both accesses. Does not impede ventilation and is not an escapeway.
Elevator had a problem with the bottom door switch mechanism from 12:12AM until 12:34AM. No injures. Was back in operation at 12:35AM.
Elevator doors being closed with excessive force causing latch to fail.
A roof fall was found on the South Mains ""D"" mainline belt inby 45 x-cut. Fall was approx. 10 ft long x 15 ft wide x 2 to 8 ft. high. Area was bolted with Install II bolts and resin assisted. The supplemental support consisted of bacon straps and wire mesh screen. Cause of the fall was deterioration of the mine roof through weathering.
Hoist was down due to blowed hydraulic filter housing.
A roof fall was discovered in the 1 north mains, crosscut 51, #6 entry. The fall measures approx. 24 feet long, 30 feet wide, and a feet thick. The area is supported with 4 foot fully grouted resin bolts.
An untentional roof fall has occurred in the 3rd East Main no. 3 entry outby 90 ft inby survey spad no. 2431. The fall measured 180 ft long, 18 ft wide, and 5 ft thick. The fall was above the anchor of installed roof bolts. The fall does not impair air movement, or impede travel in the area.
The slope car was taken out of service to repair the outby wide brake on the master slope brake car. The car was back in service at 6:00 pm on 12/15/2008.
Fire started by a backdraft or chimney draw on 2 SDRII plastic pipes. One which was an 8"", was being moved and the opening of the 8"" plated. Fire was pulled up thru the 14"" & 8"" pipe as well. This is odd, but possible, so we have started the practice of removing the existing pipe when welding or cutting on the kill box,
A roof fall occurred on the No. 1 section in the #3 entry cross cut at spad 7308. The roof fall measured approximately 25 feet long by approximately 22 feet wide, and approximately 15 feet in height.
Unintentional roof fall discovered. Fall located on No. 4 track, No. 12 block, 1 block left of station 350. Previously supported with 6' torque tension bolts and screen. Fall measured approximately 2'L X 18'W X 2'H. Area developed on or about 01/31/2005. Overburden about 260'.
Root cause analysis investigation showed that the as-built pipeline and design differed, and that this area was to have a steaper slope than designed to still allow for adequate pipe cover. The slope modification did not occur and the GPS did not incorporate the slope change.
An unplanned roof fall (highwall failure) occurred at the mine fan portal area. The fall was found at approx 9:18 PM and it was determined that ventilation was impaired at approx 10:35 PM.
The Cedar Creek elevator was out of service from 6:15 am to 8:30 am due to a problem with operating in the inspection mode.
The #3 coal mill system explosion vents opened at 8:06 PM due to an increased pressure created by an ignition of coal dust. High coal mill temperatures occured prior to the event which resulted in coal burning in the inlet duct of the coal mill. There were no personnel injuries or equipment damages.
The miner is not sure this is reportable. The hoist car was inoperable due to the batteries being discharge for the brakes on the hoist car. This problem was discovered at approx 10:05 pm. The hoist was pulled out manually and batteries changed out and back in service at approx 11:00 pm.
After evening shift went down at the start of their shift, the elevator went down and electricians went to work on the problem right away. It was discovered that the governor rope needed to be tightened up. Noone was on the elevator at this time, back up and running at 1745.
IGNITION- As the electrician was burning weld from the H/G drum, a spark from the torch fell into a crack in the footwall and set a bleeder on fire. The flame lasted 6 seconds and was extinguished with water. No one was hurt nor was their any property damage incurred.
The fall was approx. 70' long, 9' high and 19' wide (fault area).
A grass fire occurred in Sandow Mine on Friday, December 12, 2008. The fire was started at approximately 4:10 PM on adjacent property by a land owner who was mowing on his property with a riding lawn mower which caught on fire. The land owner called the Lexington and Blue fire departments. No injury resulted from the fire, and it was under control at approximately 6:00 PM.
Coal rider at # 6 crosscut dipped in above anchorage roof bolts causing roof fall. 18' to 20' wide, 15' to 18' long & 4' to 6' high.
Production hoist tripped to manual at 7:05 am 17 ' from the collar. Notified MSHA at 7:30 am. Evacuated the mine at 7:35 am. Was able to reset in manual and pull the load. Ran test trips on the hoist. Hoist operational and cancelled the evac at 7:40 am. Case # 1-59024526
A roof fall was discovered in the 2 north 1 mains at 9 crosscut, between #1 and #2 entries. The fall measures approx. 60 feet long, 18 feet wide, and 5 1/2 feet thick, the area is supported with 5 foot fully grouted resin bolts.
A methane ignition occurred in the 8th East development section. The left side roof bolter operator drilled a hole to a depth of 17"" in the mine roof, when his drill steel plugged (wet drilling). He was retracting the steel from the hole to clean the plugged steel, when the hot drill bit ignited a pocket of methane in the drill hole.
A roof fall occurred in the #6 entry in the 12 south area at x-cut #56. The fall was about 19' by 20ft, 5-5ft high. The area was bolted with 5 foot torque tension bolts. It was timbered and dangered off, it will not be cleaned up.
A roof fall occurred in the #2 entry in the left return of the 3B area x-cut outby spad #46437. The fall was about 19' by 240 foot abd 5 to 8 feet high. It was bolted with 5 torque tension bolts. The fall was timber and dangered off, it will not be cleaned up.
15 X-CUTS OUTBY #5 UNIT DUMPING POINT, #5 ENTRY SPAD 22+85 L 40' X 30'W X 8'H
A roof fall occurred in the #5 entry in the 12 south area at x-cut #60. The fall was about 19' by 20ft and 5 to 8ft high. The area was boltes with 5ft torque tension bolts. It was timbered and dangered off, it will not be cleaned up.
2,000' from section, entry 4, spad no. 604; no eqip. involved; Neutral-airway; Size 10' long, 19'wide, 4'-7' high; 48"" resin, 3 cribs, type of roof support, 3 cribs prior to fall; Strata is shale, sandstone, and laminated; Mo coal rider seam; entrances are not breakered off, roof is being rehabilitated with clean timbers, floor the cribs than the roof.
At the end of shift as the driller was backing his drill away from a heading, he noticed a fire. He exited the machine and found the flames too intense to activate the onboard fire suppression system. He ran to an equipment parking area, sounded the fire alarm, the mine was evacuated without incident or injury. MSHA one call was initiated, the mine was barracaded and attended.
Unintentional roof fall discovered. Fall is located at 11 Zone, 21 Block, No. 2 entry, 1 block inby station 829. This area was mined on or about 10/18/2006 and was supported with 6 foot resin grouted roof bolts. Overburden is about 180 feet.
Unintentional roof fall in belt line from SS# 3854 Inby, approx 200 feet.
A fall of roof occurred in the 1st East Intake, # 10 cross cut, # 6 entry, at station 3+90. The fall was approximately 35 feet in length x 30 feet in width and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.
At approximately 1:35 am the belt crew was moving belt when they heard a roof fall. Supervisor immediately went to the last open x-cut in #8 entry at #23 x-cut of 12th West section and found a roof fall 20' x 20' x 4.5' high. The area was dangered off and called to the shift foreman. Shift foreman notified superintendent and safety supervisor. Not above 5' bolts being used.
Roof fell on the 9F section #1 entry, between #11 and #12 breaks. 35'L x 23'W x 12'H. Area has been breakered off and has not affected the ventilation. No injuries or entrapment. Fall is outby the tailpiece.
Unintentional Roof Fall:20'x20'x6"", #13 crosscut 8' inby spad 502, and in the no.6 primary intake entry for the West Mains along B-beltline. 6' torque tension rebar was the primary roof support in the affected area. The fall will be cleaned up.
An unplanned roof fal occurred in the #130 mine at # 6 brk on # 5 belt that measured 60' x 18' x 8'. There were no injuries reported. The area was developed on 1-20-06.
A roof fall occurred in the Main West No. 6 entry (intake) at spad No. 5+40. Length 30 ft x 20 ft wide x 12 ft high. No equip or ventilation affected.
Roof fall in #5 entry (intake & primary escapeway) at X-cut 67 of 2-Right Sub-main, approx. 18' wide, 50' long above roof bolt anchorage (60""). No one injured, no equipment involved. Ventilation & Primary escapeway re-established in adjacent entries.
A roof fall was dicovered on 12-09-2008 at apprx 7:00 am. The fall was located 2 breaks outby spad 743 near the #5 beltline. The fall measured apprx 18'W by 50' long. All entries of the roof fall area was cribbed or timbered to prevent travel. 4' resin bolts were used at the location of the fall.
Our examiners found a roof fall during their weekly examination of the 4 West return air courses. The fall was located in #2 entry and extended into the crosscut toward #3 entry at crosscut 116. The fall was located in an entry adjacent to the examiners route of travel. The area was timbered.
A bounce occurred adjacent to the stageloader caused rib material to fall which knocked out rib support. The rib support and loose rib material fell into the walkway and stageloader stopping production.
Rock fall occurred in 1 North Mains in the #6 entry between break #47 and break #48. Fall measured approximately 20' wide x 80' long x 6' thick.
Unit was on pillars, had a fall that caught 2 mobile roof support units.
In the early am hours on 12-8-08 a roof fall occurred at the intersection of 9e + 9s below the anchorage point. In the early am hours on 12-9-089 more of the same area fell above the anhcorage point. The area has been blocked off and its abandoned. Reported to other ee on 12/10/08.
A non-injury accident, the unintentional stopping of the elevator at Camp Run Portal, entrapping one (1) person on-board the elevator occurred. The source of the malfunction was due to a loss of hydraulic fluid in the cylinder to the gripper.
ROOF FALL 18' X 80' X 7' #5 UNIT INTAKE SPAD 33+00 RIDER COAL SEAMS
Return entry was being traveled with federal inpector when observed by an employee at 1st left off of #1 SE mains at survey station 604 #1 entry.
Rock fall occurred on 1 East Mains in #7 entry at break #20. Fall measured approximately 18' wide x 30' long x 3' thick.
A 2-yard LHD bucket was slung under the Chippy Hoist skip being hoisted up the Jewell Shaft. A guide shoe came loose causing the bucket to rotate into and tearing out 2 shaft guides causing the bucket to lodge into the timber. 4 shaft crew members riding inside the skip were uninjured but briefly stranded before being transfered to the adjacent Double Drum Hoist skip.
Unintentional roof fall in #158 Crosscut #2 intersection.
The Bailey portal elevator was inoperable due to an electrical issue with the Emergency stop button. An investigation was conducted, the E-stop was replaced.
A roof fall was discovered in the 2 north 2 mains crosscut 67. #6 entry, the fall measures approx. 20feet long, 16feet wide and 4ft thick. The area is supported with a 4ft fully grouted resin bolts and 8ft of cable bolts.
An unintentional roof fall fell on the 4C travelway at x-cut 85 station 11+90 entry #7. The fall did not affect ventilation or cause personal injury. The fall did affect the secondary escapeway. Stoppings were set over and the escapeway was rerouted. The fall measured 30 feet x 20 feet x 6-7feet high.
A roof fall was discovered in the #2 entry between #6 and #7 crosscut just outby spad 76. The fall is apx. 20ft long x 6ft high x 18 ft wide. No persons nor equipment were affected by the fall and it has been dangered off to possibly clean up at some point in the future.
At 8:10 am an unplanned roof fall was discovered measuring 30 ft in length x 20 ft. in width x 8 ft. in height. This area was dangered off and will be rehabilitated. The fall was called in and investigated by MSHA. Area of fall # 1 entry, 1 brk over from SS # 3153 or 1 brk inby SS # 3123.
A fall occurred on the #1 Unit, last open in the #6 entry, crosscut 33, spad 23+20. The fall is 30 feet long, 18 feet wide and 8 feet high.
The hoist rope for the Flat Creek Main Emergency Escape Capsule broke during a functionality test. Ice had accumulated on the guide ropes which stopped the capsule's descent, allowed slack to gather in the rope, and allowed the capsule to drop the corresponding distance of the slack in the rope, which resulted in breakage of the hoist rope.
An unitentional roof fall occured on the 4C travelway at 49 crosscut station 13+30 entry #7. The fall did not affect ventilation or cause personal injury. The fall was in the secondary escapeway but their are 2 entries and the escapeway was rerouted to the other entry. The area had been bolted with 6 foot fully grouted roof bolts. The fall measured 25'x 18'x8'high.
A roof fall occurred on #3 unit, cast open crosscut between #3 and #4 room. The fall is approx 20' long, 17' wide and 6' thick. The area is supported with 5' torque tension rebar bolts.
Automatic hoist tripped to manual operation. After cycling power, the hoist rest. Ran several test trips and put back into automatic operation.
A floor heave event occured in 2nd East Mains between crosscut 6 and 7.
The above mine has experienced a non-injury, unintentional roof fall at cross-cut no. 36 thru a cross-cut between no. 3 and 4 entries along no. 3 belt mains. The fall was discovered at app. 4:30 am during a pre-shift examination. The fall is app. 5 feet thick, 20 feeet wide and 45 feet long.
The switch on the elevator brake was out of adjustment, chief electrician was called in from home and it tool some time for him to get to mine site.. The elevator was operantional at 0145 on 12/5/08.
A roof fall occurred in the #9 seam in the 1 North at crosscut 107 in the pillarline between #6 entry and #7 entry measuring approximately 20ft. x 19ft. x 4.5ft. high. This area was supported with 5ft. fully grouted roof bolts.
Fall inby spad # 874 in old works, blocking DPP #4 on Northwest Mains. 20 Ft long, 8Ft wide, 5-6 ft. thick.
Outburst in tailgate and closed secondary escapeway
@2 unit, #5 entry, miner operator reported he saw a small orange glow and heard a noise ar the face. Investagation by company, State and MSHA and it was determined that it was a sulfar ball. No methane ot dust issues in the area or with the mine
While cage was taking crew down shaft a loose handrail clipped the bottom of the cage door. Door pulled off of its tracks toward shaft, hit next landing and forced door into cage striking 3 EE's. 3 EE's recieved first aid only. After getting employees off cage it was down for more than 30 minutes to investigate.
At around 4:55 pm the Hart street elevator failed to operate properly. The elevator failed to go up and down in the shaft. The investgation revealed that the brake coil failed and had to be replaced. After the coil was replaced the elevator was returned to service.
A fall of roof occurred in the 2nd left panel off the 1st east at station # 0+90, entry # 0. The fall was approximately 40 feet in length x 20 feet in width, x 10 feet thick. The area was supported with 6 foot fully grouted roof bolts.
The resistor for the elevator brakes was open. Had to order parts. No one was on the elevator at this time, corrective action, resistor was ordered as well as spares. Elevator was opertional around 9:00 AM on 12/3/08.
One of our examiners found a roof fall during his weekly examination of the 2 Main North seals. The fall was located at crosscut 12 of the 2MN from entry #1 to half way between entry #2. The fall has blocked access to the #1 seal.
A roof fall occurred in the No. 2 entry at crosscut No. 26. No ventilation or travel of miners was blocked.
A roof fall was reported at 8:15AM on 12/3/2008 at x-cut 104 entry 5 main south. The fall was in a four way intersection that had been mined in November of 1991. Intersection was bolted with 6' resin pins. No injury or property damage resulted. There are no plans to clean up this fall. Reported because it was seen from travelway.
An unintentional fall occurred in the 2nd main west entry's at the 24+50' between the #5 and #6 entry's. The fall was 8' high by 18' wide by 25' long. Fall was due to deterioration of roof above bolt anchorage.
A fall of roof occurred in the 3rd Northeast Mains in # 2 entry, spad 10+50, x-cut 15. The fall was approx 18ft long x 19ft wide x 8ft high. The cause of the fall was determined to be an undetected void in the mine roof with water contributing.
Found a fall 1 break outby spad 2252. Area had been cribbed off about 3 weeks ago. Fell behind cribs. About 6' thick, 18' wide, 30' long, between #5 & #6 entry.
Hoist drum brake locked up while hoistman was running throug his pre-shift check. Hoistman tried to reset computer, would not reset. Electrical relay failed to operate properly. Screw on contact cross bar was loose.
A roof fall was discovered on this date. The fall was located on C-Mains, Break #17 in the #5 entry. The fall measured 20' by 20' and was 8-9' in thickness. The area was previously supported with 5' HY-tech bolts with supplemental 8' superbolts. There were no injuries or equipment involved.
Fall occured at crosscut 25 between 6 and 7 entry taking out a return stopping. The fall was approximately 40 feet long and 5 feet high.
Fall discovered 12-02-2008 inby break 18 on 2A belt in main left return. The fall was 20 ft x 100 ft x 7>8 ft high. The area was already dangered, it was breakered off, and will not be rehabed. Both MSHA & OMSL were notified.
A roof fall was found on 12/2/08 at 10:35 at x-cut 47 entry 5 of the main west. Fall was 20' x 20' x 10' high in a 4-way intersection. Area was mined September 2001 and had been supported with 8' resin pins. No property damage or injury resulted. At this time there are no plans to clean this fall up.
While mining in the # 5 entry of the 001 section, cut into old works. The size of the cut through approx. 2' X 4'. The mine is blowing ventilation therefore no ventilation issues with cut through.
A death of an employee occurred on or in the Ohio River. The death is still under investigation & the autopsy report has not been released. The cause is unknown at this time. Report is being filed under protest due to jurisdictional issues yet resolved. The deceased was working on empty barge when last seen. Upon his disappearance, a search commenced & his body found approx 2:30am.
Victim was in operation of Genie S-85 aerial lift when the lift became caught on exterior wall girt of building under construction. In an attempt to free the caught basket the machine's boom reacted after the basket came free, resulting in the victim's head receiving injuries which were fatal.
Owner of J&P equipment cleaning was working on a steam cleaning engine when one of his employees noticed that he was slumped over the engine. This is still under investgation by MSHA.
Poor communications led to the employee working near energized equipment. The employee drew an arc resulting in electrical burns to the abdomen and right hand resulting in the loss of the right index finger. The employee also received flash burns to the face.
Employee was changing batteries on Scoop when jumper cable slipped. He reached to grab cable and caught his left hand between battery and frame.
Employee was cleaning out vertical auger when he accidentally hit the ON switch while leaving his hand in the clean out area. Amputated Index & Middle Finger of right hand
Employee was shovelling debris off of a walkway when he reinjured a hernia that had been repaired before he started work at this company.
EE was in the process of doing maintenance on the E. underflow pump located in prep. plant. Before it was started, power to the motor was to be knocked, locked, tagged out. He removed the belt guard from the underflow pump; grabbed the motor pulley with his right hand while it was still running causing the r. thumb to be amputated below upper knuckle.
Employee caught right thumb between hydraulic jack and ATRS.
Carring timber in low area of mine when he felt a pull in the left testicular area.
Under investigation --- PLEASE NOTE: Injured was a contractor --- injured contractor would not provide last 4 digits of SS# or DOB. Per call for ampulation tip middle finger(75), ring of 1st joint(60) all together two are 135(left hand).
Someone in tower turned belt on and did not know he was in the area.
Driver grabbed tail gate pin and attempted to pull out on gate. Loose coal fell out taking pressure off gate causing gate to rapid shut and catching his right hand ring finger, causing amputation at first joint on finger.
Injured slipped and fell while working on locomotive in motor pit.
Injured went through a man door at #27 8R section. She tripped on some stopping block pieces in the walkway falling to the floor striking her right forearm. After working 01/02 and 01/03/09 She went to the doctor on 1/05/09 where they are treating her for contusions to the right forearm.
Trying to get truck started - slipped, fell and landed on his back on the ground - sprain/strain lumbosacral -
EE got off Damascus Scooter, failed to set brake, scooter rolled back and came in contact with left ankle.
Changing scoop batteries on S&S scoop. Smashed & broke left ring finger & middle finger.

Injured employee was in the process of installing permanent roof supports. While removing the two piece drill steel from the mine roof, one of the two piece came apart and struck employee in the mouth, resulting in a laceration that required several sutures and dislodged some teeth.
Employee was struck by a piece of drawrock on left arm and elbow. He continued working until 1/16/09 when the doctor removed him from work.
Plant Operator was clearing the grizzly bars one foot slipped between and banged his knee. He continued work for the day. He informed management of his injury on 1/8/09. Dr determined it was probably a sprain. He did physical therapy was told by Dr that his planned vacation would be sufficient recuperation. After vac. he started to miss work. Possible surgery required
Employee was getting off the seat when a rock fell out and hit the ATRS, then hit the employee in the left foot just below the ankle. It broke one bone in his foot.
Employee was making routine examination of No. 6C Sycamore Seal, he slipped and fell fracturing his left arm/wrist.
Employee was operating ram car when he ran over a rock injuring lower back. Did not start losing time until 7/17/09.
Injureds co worker struck his foot with a rail. When injured twisted suddenly, straining his middle back area. He went to the hospital where he was treated for middle back pain.
Employee was in the underground parts hole trying to lift a miner shear jack onto the two man trip. He felt a sharp pain in his lower back and lower extremities. Employee did not go to the doctor that day. He went on January 13th, 2009.
The EE was walking along the longwall face and felt a sharp pain in his right knee. EE began loosing time as of 1/12/09.
Employee was entrapped in limestone dust and injured right arm while being pulled out.
Conditions contributing to the accident are unknown. Victim has broken ribs, broken collar bone, and a collapsed lung.
Employee was checking stuck roller glove got caught pulled hand and arm into roller and belt.
Employee was crossing the hood of a rock truck and tripped on a shovel and fell hurting his back.
EE bent over to get box glue to put onto RB. Draw rock fell, striking mid back area. He was transported to ER, referred to neurologist. 1/13/09 EE saw neurologist, who took him off work until 3/2/09.
The employee was pulling a loose rib with a slate bar. As the rib fell, more rib fell than he had anticipated, hitting the slate bar causing the employee to twist his lower back. The employee was reffered to a specialist that ordered a CAT Scan. The specialist on Tuesday 1/6/09, after getting the CAT Scan results, released the employee to return to work on 1/12/09.
Employee slipped and fell flat on back. No treatment until 5/8/09. Injury determined, was torn muscle in shoulder, surgery required.
Injured was helping set an H beam. As the beam was slid, it twisted striking him on the knee causing him to twist his right knee. Note: He worked until 5 am & then went to Dr. He was given a release to return to work by an orthopedic specialist & scheduled for an MRI that was negative. EE worked until 4/28/09. He was taken off work after another scope was done.
Injured EE was walking around a continuous miner, when EE stepped on the miner cable that was lying on the mine floor, causing EE to loose his balance and straining his right knee.
While driving out of the pit and going up a hill the vehicle started spinning tires on the ice so the employee got out of the truck to see if the 4-wheel drive was engaged. When the employee exited the vehicle he slipped and fell on the ice.
DID NOT START LOSING TIME UNTIL 04/30/2009. SURGERY ON 4/30/09. Jacking dust tank, slipped and felt pain in right knee.
Employee slipped and fell on back while walking on the county road to #4 Dam. The road condition was wet and slippery from a recent snow storm.
On the day of the injury, the employee was at the site early to start equipment up for the start of the shift. He had just started the D5 Dozer and was attempting to egress it, when his right foot slipped off of the equipment provided step. His left foot made contact with the ground and twisted, as a result, a bone in his left ankle was broken. (sprained not broken per MM)
Miner was riding mantrip and the mantrip backed into a timber, it fell and struck the miner's lower leg.
Employee was installing a rock bolt with a jackleg. While lifting up on jackleg in order to reposition leg back a few inches, the exertion caused his knee cap to pop out of place.
Spraying down equipment with water hose. Twisted lower back when he went to turn around.
Employee was walking to ready line and slipped on a patch of ice injuring rear and left side of body. As of 12-28-08 this became a lost time accident.
Employee was operating the longwall sheraer when he placed his right hand on the hand rail a piece of rock flew from the drum and struck his right ring finger causing a fracture.
While setting 4 x 4 timbers, one fell, cutting his upper and lower lip and breaking a tooth.
Employee was cleaning roof bolter when he fell & hit his back on the canopy.
He was assisting the center bolter while installing bolts. He reached across the bolter head to grab a stuck steel, his left foot was under the mast when lowered, causing a crushing injury to the left foot.
EE was operating a shuttle car when he felt an irritation in his right eye. When he stopped, he took off his glasses and gloves and rubbed his eye. He then flushed his eye with water. After his shift he went home and went to bed. When he woke to go to work his eye was still irritated and swollen.
Slip/fall on icy road; at approx 6:30pm, EE left the dining area and was walking to the STP to start her night shift for that evening. As EE was crossing the road, she slipped and fell on the ground. She realized she had injured her ankle and she immediately reported to the medic.
Crew was in wash bay taking out screen deck & mill grates on the secondary crusher. Injured got is right index finger pinched while helping remove screen parts. (Hammer Mill grates) Was pulling a grate out by himself & weight of grate shifted & pinched finger. Stitches in right finger & surgery to mend tendons.
Employee slipped on last step of loader and fell to ground. Icy conditions contributed to the incident resulting in a bruised back.
Hammering a block of stone, hammer missed and hit himself on the left shin, did not request a doctor's appt until 12-31-08
Employee slipped on patch of ice and his right wrist. As of 12-30-08 this became a lost time accident.
EE was holding plastic around hyd hose. Hyd oil was injected into index finger. (Did not receive this report of injury until 12/31/2008).
The employee stated he was bolting and was pushing the bolt wrench onto the bolt and had his hand on the bolt when he accidently hit the rotation on the machine. His glove was caught between the wrench and the roof bolt and this caused the glove to the pulled off his hand and hyperextended his fingers.
Employee was removing a damper blade from kiln 2 scrubber and pulled his lower back. He didn't go to the doctor until 12/31/08 and was placed on restricted work duty.
Was getting onto machine, foot slipped and leg was caught in track and twisted knee.
Employee was running helper side of bolter. Bolter tire ran over his left foot.
iner was walking across plant site during heavy snpw/ice weather, slipped on ice covered pavement and fell to the ground striking right shoulder.
Employee stated that while taking down curtain, he twisted the wrong way and hurt his back.
Employee was capping top of can, when he stepped down off ladder he stepped on piece of wood rolling his ankle.
employee stepped on a rock while walking her left foot rolled. resulted in a fracture.
WHILE CLEARING SNOW FROM A STAIRWAY, WORKER SLIPPED ON ICE/SNOW AND HIT HIS SHOULDER ON HAND RAIL. THIS INJURY DID NOT BECOME REPORTABLE UNTIL 1/28/09 WHEN, AS HIS PAIN WORSENED, WORKER WENT TO DOCTOR AND WAS DIAGNOSED WITH A TORN ROTATOR CUFF. WORKER WAS ON SEASONAL LAYOFF AS OF 12/24/08.
Employee stated he was descending stairs, stepped from stair to floor, lost his balance and he fell causing him to sprain his ankle.
Employee got a blister on his left leg from crawling. It got an infection from that.
Breaking bolts loose on starter and felt something pop in hand.
EE and another EE were moving 50lb bags of magnatite. He said he felt a twist, thought he must have pulled his back. Worked a full day and did not report until following day.
Employee was replacing bolt on screen when counter weight pulley pulled bolt & employee's left hand onto pulley under belt. Employee was pulling bolt when injury occurred. Center finger on left hand received 3-4 stitches.
Employee was working in panel. He reached up with his arm. A piece of the roof came loose and smashed his fingers on right hand.
Lifting a timber to load in scoop bucket, had sharp pain in lower back and fell to floor. Employee put on back board and took to hospital where he was treated and released then returned to work next shift. On Jan 5th, 09, he called in with back hurting again and went to dr and has not returned to work since.
Employee stated he was undressing at his locker preparing to put his work clothes on and while hanging up his ball cap, his finger came in cintact with the locker door and cut his middle finger.
While removing ball cone from #3 ball mill, the lifting cable broke striking injury on his right hand causing a contusion to that area. He was prescribed a wrist support for the affected area.
Pinched left index finger while positioning a wedge between a jack and the jeep that was being re-railed.
Injured was pulling a cable when he stumbled over an unseen rock. This caused a contusion to his left big toe.
Employee claims he received an injury to his wrist as he attempted to dislodge a jammed steering device on the RT dozer.
EE stated that he was in the process of roof bolting the face of the #2 entry on the #1 unit. While pulling the drill steels back out of the roof, one of the drill steel sections stayed in the mine roof. While the ee was placing drill steel sections in the roof bolter's tray, the steel that had stayed in the roof fell out striking the ee's finger on his left hand.
Injured was helping cm operator when a piece of roll fell striking injured on back and head.
Individual was getting out of truck, dropped his lunch box, went to catch it and caught hand between steps of truck - fractured hand
A chunk of Gilsonite off the slope, striking him just below the hard hat, behind his right ear.
Miner was bending a metal pole and it broke down resulting in the person to fall breaking his legs.
Employee stated that while lifting a shive screen, he injured his abdomen and right testicle.
An over-nite ice & freezing rain storm, made all areas slick and very hazardous.
At end of bagging run, the material gate wasn't closed causing the blending hopper to fill up. The slide gate switch was manually held on for gate to remain open so that the hopper could be cleaned manually. Mis-communication between the people cleaning the hopper and operating the switch resulted in the gate closing, causing it to cut off the tip of the finger.
Traveling down haulroad in loaded truck, driver thought he was in the wrong gear and got truck out of gear. Driver applied brakes. Truck wouldn't slow down enough to get it in gear. Truck made left hand turn. The truck tipped to the right, the driver over compensated and the truck turned over on left side.
The coal truck driver, just finished loading his dump truck with a front end loader and was exiting the loader via the ladder when he slipped and fell onto the coal pile. He was not aware of any injury at that time. On Tuesday, January 6, 2009 he was taken to the emergency room and after evaluation and tests he underwent an emergency splenectomy.
Cutting metal platting plate flexed upwards strking miner in the face injuring nose, and mouth area requiring stiches.
EE stated that while hauling a load of coal to the prep plant, he hit a hole in the roadway, injuring his back.
Employee was operating the roof drill, he alledged that he pulled the wrong lever and the drill head hit his right leg.
EE was working on the wiring harness and the wind blew the door shut, hitting him on the head and caused him to cut his lip.
Employee claimed he hurt his wrist climbing down from truck on 12-19. He did not report until 12-22. Said he would be off 2 weeks. Our Dr. returned him to work to drive truck on 12-31. Investigation found employee had personal surgery and did not report to company. Under investigation due to holidays, will follow up and report to MSHA. Company at this time is denying accident.
While trying to secure a 30"" SDR-17 pipe into a fusing machine, the pipe came out and over the ""jaws"" of the machine, striking the worker on the right side.
An employee was struck by a rock which dislodged from the mine roof. Ventilation tubing was needing to be extended and the employee stepped up to the miner to retrieve a drill so a spad hole could be drilled to secure the ventilation tubing. As the employee stepped back away from the miner, the rock fell from the roof striking him in the head, neck and back.
While stepping from the roof bolter, injury felt pain in his back, He completed his shift. While he was off work for the holidays. He went to the hospital on 12/26 for pain in his back, x-rays were negative and he was referred to a special in VA, who had preformed surgury on his back approx. 2 years previously.
Employee was pushing a wheelbarrow along bunker and the wheel hit a rib and then fell over on the employee's foot.
Employee was changing a transmission in a truck when he felt pain in his lower back. He continued working until 01 04 2009 when he went to a doctor.
Muddy, walking to end of tracks to get a ride out of mine. Got stuck in mud, fell and twisted ankle.
EE was assisting pulling a belt togather and he stepped over the pull cable, the chain on a sheave broke the cable striking him in the lower leg causing a break to the leg.
Employee was taking a steering jack off of the 488 scoop and smashed his right middle finger against the machine. He worked to the end of the shift. He went to the doctor the next day. The doctor told him he fractured the end of his middle finger.
Injured got on the bolter to get something and stepped off. Area was about 1' off the bottom. He twisted his left ankle falling to the bottom. Injured was transported to the hospital. He was being treated for a sprained ankle.
Employee states that when he stepped out of the truck he was driving he slipped on ice and landed on his left side.
The victim was loading supplies on a scoop bucket when a piece of draw slate fell from between the roof bolts and struck him on the left hand breaking his little finger. His hand was resting on the edge of the scoop bucket when the 1'x1'x2"" thick piece of slate fell. He was not operating scoop at the time.
Employee was making a cable pull hook for the boom of 004 Miner. The 1 1/2 inch rolled steel had been heated & a crib block against the mine rib & using the swing action of the conveyor boom of the miner to achieve the desired bend in the hook, the crib block struck employee in the thigh of his leg.
Employee was pulling shotcrete line and a chunk of shotcrete fell from roof striking employee.
Employee stated as he was dismounting the company number 262 Caterpillar bobcat he slipped and lost his footing. He struck his right forearm on the bucket of the bobcat. Employee was diagnosed with a possible radius fracture and placed in a cast.
Employee stated he was helping unload 3 ft. crib ties off of a trailer when another person grabbed one and threw it off and it glanced off of another one and hit him in the left knee. Employee has continued to work until 4/22/2009 when he was taken off by the doctor.
While working with two other employees ratcheting a come-a-long to lift a ripper claw, the chain on the come-a-long broke and the ripper claw fell and hit employee above the right knee.
employee was stepping down from a palletizer and tripped on an emergency stop cord causing him to fall against a handrail. He fractured 4 ribs.
Employee was shoveling belt when he stated that he felt pain in his lower back and his right leg was hurting.
EE bent a roof bolt during installation. The head of the bolt stuck in the chuck of the bolter. The EE was working his drill head up and down to try and free the roof bolt. He accidentally hit the rotation lever, causing bolt to spin and strike his arm.
injured was hanging shuttle car cable when he twisted his right ankle
EE was plastering stoppings when he reached above his head and plaster splashed in his eye.
Employee was breaking up a rib roll with a hammer when something hit the top of his right hand on 12-17-08 employee continued to work until 2-18-09 when he became a lost time due to having surgery to remove object on top of right hand. employee returned to work on 2-23-09 with no restrictions.
Member was walking between the rib and a supply motor when he slipped and fell on his left shoulder. Member was not taken off work until he was seen by a specialist on 12/31/2008.
The employee was using a set of torches to remove wear strips from a continuous mining machine. He was positioned under the machine and a piece of slag fell in his ear. On January 5, 2009 a physician determined that medical treatment was necessary to treat infection of the ear.
The employee was taking a scoop off charge when he fell into the scoop canopy striking his head. The employee had bruising.
While reaching to activate the control box to pull the shields in, the high pressure hose ruptured near the center of the hose. The solcenic oil sprayed EE in the face. He then tripped over a rambar and fell. As a result of this incident, EE sustained a laceration to the back of his head.
Building an overcase wall. Wall fell on employee's left lower leg and foot.
Employee thawing out frozen belt. A piece of ice and rock slid off belt smashing fingers on left hand. Fracture to little finger.
Employee was climbing down steps of Cat 777 rcck truck, he had tools in his hand. He missed the last step and fell to the ground landing on his right hip and back.
EE was cleaning up a stockpile area around the outside edge and hit a rock that threw him forward hitting his right leg.
EE was operating a ST6 Mucker and he was scaling the face, when the mucker bounced causing EE to hit his thigh on the canopy frame, causing a contusion on his left thigh.
Employee was attempting to dismount a D11 Caterpillar dozer. He placed his foot on the step on top of the tilt jack and his foot slipped causing him to fall back onto the tracks of the dozer. Note: Employee was seen by an orthopedic specialist on January 6, 2009 who on that dat took him off of work and prescribed physical therapy.
Employee was shoveling a belt line when he felt a pain in his lower back. The employee continued to work until 1/5/09 when his condition worsened and he began physical therapy.
Employee worked all day doing normal duties. He stated he woke up the next morning and couldn't get out of bed. He thought he re-aggravated his back from a reoccured injury.
Employee was injured when he slipped and fell when he was climbing into his drilling equipment. Snow and ice conditions on the steps caused the employee to slip. Employee was trying to get an insert out of a roof bolter wrench. As he struck the wrench on the metal lip on the side of the bolter machine, he struck his left index finger between the wrench and lip. Injured had a laceration that required stitches and a fracture.
Employee experienced strain in lower back after the front end of his grader was struck by a haul truck.

Employee was in the process of shutting off a water pump, he stepped up on the pump and lost his footing and fell backwards landing on his back, He bruised and sprained his back.
EE stated that while roof bolting in 2 South East, he was trying to pull a roof strap back to line up the hole to install a roof bolt. When doing so, he felt pain in his right shoulder. After further investigation, it was diagnosed that the EE had a muscle strain and on 12/9 it was determined that the EE missed time from work due to the condition and is now being reported.
He was kneeling down and felt pain in his left knee, diagnosed with left knee strain.
Employee was hauling dust to a unit when the ram car he was operating hit a steel set leg causing the horizontal beam to fall striking him on the head and neck.

Employee was unloading a water pump from the bed of the F350 truck. The truck bed was slick due to rainy weather, therefore as he was descending the truck bed with one foot out and one foot in, he slipped on the wet surface falling to the ground. Twisted his right ankle, resulting in it swelling.

EE was greasing a loader in the shovel pit. As he was dismounting the ladder he stepped on a rock at the same time he let go of the ladder which caused him to fall and twist his right ankle. EE was taken into the ER to be checked & he was taken off work.
EE was working on the 3rd floor of the Screening Plant. He was at the sizing grid and stepped on a piece of floor grating. The piece of grating gave way and EE fell approximately 12 ft. to the floor below.

EE was operating an All Terrain Vehicle in an unauthorized & unsafe manner(jumping a berm). The ATV flipped over forward, landing on the EE who sustained a broken leg & dislocated hip as a result. The EE had been instructed not to operate the ATV. The ATV was inoperative until the injured EE & another EE made the ATV operative against the supervisor's instructions.
EE was welding a fitting in a tank from a water truck, there was a spotter outside in the tank. The EE took a break and left a propane torch hanging in the tank. When he returned, struck the lighter in the tank and there was an ignition. Post investigation revealed that EE had left the torch valve partially open and had not turned on the fan prior to reentering the tank.
Employee was underneath Cat pushing starter up in place. He experienced pain in his back and legs started to go numb. He came into dispatch to report his incident and the guard gate was called to bring him in for medical treatment.

While mounting haul truck the miner missed the top handhold on the ladder causing him to fall to the ground.
Employee was removing a support pipe when the pipe unexpectedly moved and the employee grabbed the pipe. He felt a strain in his groin. He was evaluated by a physician on 1/13/09 and diagnosis with a hernia. He started to lose time on 1/21/09.
Employee was at the discharge chute of the raw coal screen removing trash from the grizzley when he observed a feeder flight coming across the raw coal screen. He tried to catch the flight to keep it from going into the heavy media vessel. The flight was too heavy and caused a pain in his left elbow.
Injured was walking #2 entry through a water hole 6""deepx18'widex20'L when he twisted his left knee. He was transported to the hospital where he was diagnosed with a sprained left knee.
EE was attempting to align the roof bolt coupler through the roof strap when the miner rocked as the operator sumped back causing coupler to bind in the strap and bend the roof bolt catching his R hand between the plant jack & roof bolt. EE was wearing West Chester Gloves. EE sustained lacerations to the index, middle & ring fingers on his right hand requiring sutures.
The EE was operating the 7LS Joy shearer on the 000 west longwall face, when he struck his head on a low shield. The EE felt pain in his lower neck.
While working at mine, wind blew pumice particle into eye. Did not go to doctor right away. Went to Dr. Johnson in Malad who referred him to Country Hills Eye Center in Ogden, Utah where they numbed the eye to remove the pumice particle and gave antibiotics on Dec. 9, 2008.
Employee reported: When climbing down light plant stationary ladder, shoulder popped. He didn't slip, felt his body weight caused the shoulder to pop and experience pain.
While installing pins a rock struck the back of his head knocking him into the boom of his pinner cutting his eye.
The employee was using a Die grinder to widen holes in a truck bed liner floor plate. The employee was bringing the tool up out of the hole when the grinder slid across the liner plate and hit his left knee causing a small puncture wound. He was treated and initially returned to full duty. At the present time the employee is under observation of a physician for infection.
Injured worker was walking from Main office to Training Trailer, slipped on snow covered ice, and fell hard to concrete with bodyweight landing on right leg and foot.

He was lifting the end of the tie to clear the bucket tooth & other ee was pushing the ties into the bucket when the his finger was pinched between the tie and the bucket tooth.
Setting a rib timber a piece of rib fell off and lacerated the miner's left ear.
EE was roof bolting on MMU No 002 section. EE placed metal strap across ATRS on roof bolter. A piece of rock approx 2' X 2' X 4 1/2"" thick fell, striking EE on top of the head.
Employee stated that he was lifting top idlers for the belt set up and loading them on a skid. When he tried to lift a roller, two rollers were struck together, causing a sharp pain in his right shoulder. Employee's FLWD did not occur until 1/7/09 due to surgery.

Employee was carrying a portable propane tank when he slipped and fell. Employee has a cut on his right index finger and broke his right leg.
While pulling a piece of conveyor belt with a pair of vice grips, the vice grips slipped off the belt causing the employee to fall backwards onto his right foot resulting in a fracture to his lower right leg.
Employee was walking through an area that was muddy from an accumulation of clay mixed with rocks to close hopper doors. Employee lost his footing and his right foot slipped from under him. The employee twisted his right knee.
Stepping off of haul truck slipped in mud twisted knee resulting in knee strain.

During shift change employee was dismounting the bus and slipped and fell on the bus stairs. He landed on his elbow which ""jammed"" his shoulder.
Employees were attempting to clean rocks from punch plate holes, striking rock with a hammer causing fly rock. A piece of rock struck injured on nose. He did seek medical attention; however, when he began having difficulty breathing decided to be checked by the company Occupational Health doctor. Forwarded to an ENT for further evaluation. Surgery 2/11/2009.
While spinning a bolt during installation, a small piece of shale fell and was struck by the spinning plate, causing it to strike the operator on the right side of the face leaving a laceration. Transported by ambulance where the wound was cleaned and sutured. Advised not to return to work until his risk of infection is reduced, minimum 1 to 2 weeks.
EE stepped back towards jeep from inside drive area, his foot slipped on a piece of belting on the roadway, causing pain to his right knee.
EE was unloading a drill bit that weighed approximately 130 lb. off the back of the truck onto a pallet. As he was throwing it on the pallet, his right glove got caught in the butterfly valve, taking down the employee as if fell to the pallet. EE was taken to the ER to be checked. He was released to work on a restricted duty status.
Employee was removing stone from conveyor when another stone hit his left finger causing a fracture; Restricted duty assigned. **Employee did not seek medical treatment until 12/12/08**

Employee was loading a roll of conveyor belt onto the boom truck. After unhooking the hoist from the roll of belt, it began to roll, rolled off the truck, striking ee and landing on his foot. He completed his shift that day, but went to his family doctor the next day. The doctor ordered x-rays, finding a fracture in his foot bone.
While splitting stone, a rock fell on his right leg.
Associate was assisting to move and lift a 32 foot fiberglass ladder from the scrubber building. The ladder was located in a tight area and was entangled in an upper unistrut. While one associate was working to release the upper portion, the associate on the lower side twisted and injured his lower back.
EE injured his knee when he jumped off a barge coalman app. 2-3 feet. He did not seek medical treatment until 12-17-08 and was returned to work. He went back to the doctor on 12-22-08 was given work restrictions which made this incident reportable. It is being reported within the required 10 working day time frame since he began losing time on 12-22-08.
Employee was in loader taking directions from injured. The loader was lifted up in front, and then dropped. He got out and found injured on the ground.
Employee's shift began at 7:00 p.m. on 12/5/2008. He was installing a transmission on a haul truck. A weld on a D-ring broke and a chain struck him resulting in a fracture to his right arm.
Emp. was installing new light bulbs from the 4th step of a 6 ft. ladder, as the emp. was coming down the ladder he stated he blacked out or fainted and fell to the ground. After splinting the emp. RT. ankle he was taken to the E.R for X-Rays, X-Ray was positive. The ortho Dr. performed surgery on his RT. ankle. Emp. is at home recovering until further notice.
Employee was walking down the face of a heap that was actively leached when he tripped over a large piece of rock and fell. As he fell, he attempted to catch himself entirely with his left hand. His left hand took the entire force of the fall and his forearm was broken just above the wrist. One of the two bones in the forearm broke.
Tightening bolts with impact gun, gun kicked back off of bolt, shoving against right shoulder.
Injury was operating a bull dozer and he was hooking a lime bin to the dozer so the bin could be moved. After the bin was hooked up and ready to move he noticed a rock underneath a wheel on the bin and attempted to remove it. As he did this the dozer pulled forward. The wheel went over his arm, shoulder, back, and center of his body.
While examining the mining machine for water leaks, the employee dropped the pan of the miner onto his left leg. This resulted in a fracture to the left femur and torn muscles.

EE was carrying a 5hp electrical motor for replacement on a piece of equipment. He felt a dull pain in the right side groin area and did not initially want to see doctor. Pain did not subside, so on 12 11 08 the employee decided he wanted to see a doctor. He is on light duty with no lifting more than 5lbs. Recheck on 12 19 08 - hernia.

Employee's shift began at 7:00 p.m. on 11/30/2008. Employee was a passenger in a van that collided with a truck.
The injured had finished installing his two roof bolts and was watching his buddy install his bolts. A rock fell from in front of the bolter ATRS flipped back and struck him on the left calf muscle area of his leg. The action bruised his calf muscle. He went to a follow up appointment on 12/5/2008 and was removed from work.
Employee was attempting to pull an outside door shut, and the wind caught the door slamming his right hand between the door & the door frame. On 12/31/2008, Doctor gave employee a full release. Doctor re-evaluated employee on 2/25/2009 and imposed work restrictions on use of right hand.
Strained left knee while attending barges. Employee did not report event to employer because of no apparent injury. Employee continued working until end of season and elected four weeks of vacation pay until 12/30/08. Employee claims restricted work disability effective 12/31/08.

The employee was removing a circle cutting attachment from the end of the plasma cutting torch head.EE had gripped the attachment with his left hand and the torch handle with his right hand.EE accidentally pushed the trigger on the torch turning the plasma cutter on. He received a third degree burn to his left hand palm.
Employee was opening a top rail car hatch and felt a sharp pain in his back. Employee was seen by a doctor and placed on modified duty.

Employee bent forward ( stooped over ) to pick up a net with a handle when he felt a pain in his back. The net is used to retrieve objects from a wash tank and weighs less than 5 pounds.
