An unplanned roof fall has occurred on the number one section, 1st south mains at spad number 120, approximately 5,000 feet from the face. The size of the fall was 25'x18'x5'. No miners or equipment were affected by the fall.

Primary hoist down amps showing 2300 with everything turned off. Electrician found a loose wire and the hoist was back operating at 8:30 AM.
Unintentional Roof fall that occured on HG #3 setup rooms #3 entry inby x-cut #16. The fall measured 18'wx40'lx9'h.
A roof fall has occurred in unit #3 at crosscut #42 in entry #4. MSHA was notified at 0900 on December 31 2009. The area around the fall was re-supported and all approaches to the fall were propped. The travel way was also re-routed.
A Fall of Roof occurred in the 2nd Northeast Parallel in the # 3 Entry, spad 24+56, x-cut 35. The Fall was approx 25ft long x 15ft wide x 6ft high.
A roof fall was discovered in the #1 Entry approximately 80' inby spad 172 at the Cook Portal. The fall measured approximately 45 feet long x 19 feet wide x 6 feet high. No injuries resulted from this fall.
A roof fall occurred on # 3 unit, MMU 003-0. The fall is in # 6 entry, at # 9 crosscut. The fall measures approximately 20' long, 18' wide, and 10' thick.
An unintentional roof fall occurred in the #5 entry at break #28 - 2 breaks inby spad #156. The fall measured approx. 6'H X 18'W X 45'L.
A ROOF FALL occurred 6 breaks inby survey station No. 2161 in the Bleeder entry between the Nos. 4 and 5 EP. The fall composed of laminated shale measured approximately 90' long by 20' wide X 4' + thick.
An unplanned roof fall occurred in the #2 entry in the Big Daddy Mains, 75' inby Spad 634. Fall measured 320' long x 20' wide x 8-10' thick. No equipment or injuries, passage of person was affected. Area was developed 11/02 and roof support consisted of 4' resin bolts. State and Federal agencies were notified immediately.
#1 Unit travel way (x-cut 23) Lenght 20', width 32', thickness 8'.
Roof fall found on the weekly exam at break 65 on 3 west mains in the return #6 entry of #7 entry. Fall was inbetween spads #6857 in #7 entry and spad #6252 in #5 entry. Fall was 30' long X 8' thick. Airflow was not disrupted.
The guide roller to the elevator car had the lacking to separate from the roller causing the car to shift slightly which activated the emergency brake.
A roof fall was found by the weekly examiner during his weekly exam on the 2F panel.
During the weekly examination, a reportable roof fall was found in F-Seam, entry 4 at crosscut 20. The fall was approximately 20 feet wide, 20 feet long, 6 feet high.
Mechanical Failure, elevator shut down, men sent home, reported for repair.
Roof fall in return room off 2 south mains from #2 Entry through Intersection to #1 Entry. Roof fall was around 70' in length. This fall was in the old works and there was no damage to any equipment. The area was cribbed and dangered off.
a methance gass ignition occurred on the 12 east continous miner section. no injuries or damage to equipment entailed.
a non-injury unintentional roof fall occurred while retreat mining on 001-0 working section in #1 entry on 2nd cut of pillar block #1. The roof fall wedged the continuous mining machine between rock and the coal rib. app. 3 ft. of rock on the machine, laminated shale and sandstone.
A roof fall occurred on the old 5A track across from spad #46684. The fall was 16' long, 20' wide and 6 to 7' high. The fall will be cleaned up and re-bolted. The x-cut was crib off and 2 steel beam set on the out by side, and both side were rope bolted with 12' bolts.
A roof fall was discovered at 6A longwall recovery area in belt entry @ 0 + 00 (between 3 and 4 crosscuts). Dimensions measure 60 feet long, 15.5 feet wide and 13 to 15 feet high.
An un-planned roof fall occured in the Subcamp mine at brk #26, inby spad #1622 that measured approx. 160' long X 18' wide X 4-8' high. No injuries were reported. The area was developed on 12/16/09.
A roof fall occurred on the 1 east 3 left section in the No. 2 room at intersection 1+50.
A roof fall occurred at the 3rd northwest mains #2 entry xc-24 to xc-25 spad #16+80 to 17+50. The size of the fall was length 130ft, width 18ft and height 6ft plus.
An unintentional fall of the mine roof was discovered in the #2 entry at #71 x-cut in the north west mains. The fall was approx. 7 to 8 ft high, 19 ft wide and approx. 400 feet long. The initial discovery of this fall revealed that draw rock has fallen out below the anchorage point of the roof bolts used in this area. The area was centered cribbed and dangered off.
The door switch located at the mine floor for the 8 Haulage elevator froze up due to adverse weather conditions. The elevator was down from 5:10am to 6:45am for a total of 1 hr. 25 min.
A roof fall was discovered at 270 + 85 number 10 entry (behind Blake's Ridge Portal). Dimensions measured 15 feet wide, 30 feet long and 8 to 9 feet high.
The 4400 157 heading had a ground fall of approximately 1500 - 2000 tons. The loader operating in the heading was partially buried, however the loader operator was able to exit through the back window with the help of two co-workers. There were no injuries as a result of the accident.
A roof fall occurred at the 3rd northwest mains xc-21 to xc-28 spad #14+70 to 19+60 #6 and #7 entries. The fall was length 560ft, width 18ft and height was 6ft plus.
Roof fall at No 5 entry between 15 & 16 x-cut on 2 south mains. Fall was 20' wide X 50' long X 5'-9' thick. Cleaned up fall and rebolted area. 10 day reporting window extended due to mine holiday and vacation.
A roof fall occurred at the 3rd northwest main #1 entry xc-25 to xc-27 spad #17+50 to #18+90. The size of the fall was length 130ft, width 18ft and height 6ft plus.
The Silver Shaft Hoist had an electrical malfunction. Two printed circuit boards failed rendering the hoist out of use. The electrician got the hoist running again at 11:08 pm. All crews stopped working and were hoisted out of the #2 Shaft. Our secondary excapeway.
The area where the fall occurred is a bleeder travelway for the 3 North Panel. One fall occurred in the intersection at 29 Break along the #1 Entry. The area was bolted on advance with 6' fully grouted resin bolts. Some areas around the fall were cribbed previously. Fall varies from 6 to 7 feet in height, and 17 feet wide by 18 feet long. Approximately 500' of cover.
The area where the roof fall occurred is a bleeder travelway for the #3 North Panel inby the 3N, #6 Evaluation Point and proceeded up the entry towards Spad #5685. The area was bolted on advance with 6' fully grouted resin bolts. Some areas around the fall were cribbed previously. Fall varies from 6'-7' high x 17' wide x 18' long, approximately 500' of cover.
The area where the roof fall occurred is a bleeder travel way for the #3 North Panel at the intersection #2 Entry of #28 Break of the panel. The area was bolted on advance with 6' fully grouted resin bolts. Some areas around the fall were cribbed previously. Fall varies from 6' to 7' high by 17' wide by 18' long. Approximately 500' of cover.
Upon inspection of the Cooks Branch elevator shaft, it was discovered that the elevator doors were not properly working and needed to be repaired. This shaft is not used to portal miners or supplies and is not the primary or secondary means for escapeway from the mine. Parts have been ordered to repair the doors and the mines will continue to maintain the elevator.
A roof fall occured on the 3rd Northwest intake #3 entry xc-18 spad # 12+60. The size of the fall was 25ft long,19ft wide and 7ft high.
Slope car was hoisted out of mine, when car was inspected on surface it was found that the brake sensor wires were broken and one of the brake sensor brackets was bent. The car was immediately taken out of service. Although exactly unknown in all probability a patch of ice approximately 100' down the slope snagged the brake sensor wires causing the damage to the slope car.
Roof fall in the last open cross cut of the #5 entry, two cross cuts in by spad # 5962. No injuries or equipment involved.
#3 hoist tripped on ""Drive Converter A"" and ground fault. Electrician was called out of the mine to check the hoist and reset the faults. Electrician closed a window in the hoist penthouse to prevent steam formation which causes false hoist tripping. There were still two remaining escapeways throughout the incident (#2 and #6 shafts).
Roof fall in intersection. Fell above anchorage in No. 2 entry L.O.B. Slick-n-side roof. 8"" cable bolts installed in intersection in a star pattern with 5 foot resin bolts as primary support.
A roof fall occurred on the 1st West Sub-main at spad # 6+30, entry #6. The fall 18' long X 19' wide X 6' high.
Roof Fall: Intake slope entry between cross-cut 4 and 5 approximately 40' long, 18' wide, 5' in height. No one injured, no equiptment involved. Found on pre-shift examination.
A roof fall occured at the 3rd Northwest main #5 belt entry between spad #11+90 and 12 + 60, xc 17 and xc18.The size of the fall was 35ft long,18ft wide and 8ft high.
A fire at the Pyrite Street Location took place. A hydraulic steering hose burst while A 988 loader (company loader # 360) was in operation, splashing fluid on the motor causing a small fire in the engine compartment. The fire was extinguished with in 3-4 minutes. The Riverside County Fire Department was called as a preventative measure Incident # 111914.
On 12/20/09 Excel Mine elevator was out of service due to a power surge from a snow storm causing electrical problems with the elevator.
The East Finley Elevator was down for more than 30 minutes. The elevator was down due to the control power that controls the doors had to be recycled to open the doors. There was no damage to the elevator. There was one person on the elevator at the top landing for 35 minutes.
The roof fall fell in the intersection of #5 heading. It was approximately 20'wide X 30'long X 8' high. No persons nor equipment were involved. The area has been dangered off and cross beams installed.
An unintentional roof occurred on the 4C panel, station 32+90 entry #4 in the neutral on the backside of the belt. Ventilation was not affected. It did not cause personal injury or affect the passage of men. The fall measured 18foot x 18foot x 6 foot high. The area had been bolted with 4 and 6 foot fully grouted roof bolts.
A roof fall occurred on # 3 unit, MMU 003-0, at crosscut # 9, # 7 entry. The fall measures approximately 14 feet long, 10 feet wide and 5 feet thick.
MMU N/003 section was pillaring on retreat mining in the #4 entry. The entrapment of the miner was 12 crosscut up the 3rd/lt panel off the 2-lt submains. Draw rock fell on the head of the miner causing the entrapment of the machine. (MSHA was notified by the telephone on 12/18/09).
#3 hoist tripped on ""Drive Converter A"" and ""Vibration"" faults. Maintenance foreman and electrician were called out of the mine to check the hoist and reset the faults. There were still two remaining escapeways throughout the incident (#2 and #6 shafts).
Three men were entraped in the Livingston Portal Elevator when a fuse had blown in the elevator control room. The elevator had stopped at about 100 ft. in the shaft from the surface. There were no injuries that resulted to any persons in this incident. The men were safely removed from the elevator at 10:20 AM.
A roof fall occurred in the #3 intersection at spad #50251 on the #1 section. The fall was 20' wide, 60' long, and 5 to 6' high. The fall area was bolted with 5 foot torque tension bolts. The fall will not be cleaned up, it was cribbed and danger off. The section was moved out of the area.
An un-intentional roof fall was discovered in the primary escape way of the 2nd southwest mains. Travel way was blocked but due to multiple entries the airway was not. Stopping line was set over to establish new travel way. Fall was breakered off with timbers. Fall measured 20' L X 18' W X 6' thick. Fall consisted of laminated shale.
An unintentional roof fall was discovered on the 004 section in the #4 entry between #3 and #5 break. Size of fall was estimated to be 120' L X 7-8' H X 18-20' W. The area had been supported with 6' fully grouted resin bolts. No injuries, no equipment caught or ventilation was disrupted. Fall will not be cleaned up.
A power outage occurred thereby diabling the hoist. The mine was evacuated due to the lack of two escapeways.
The Livingston Portal elevator was removed from service for a bent safety rod. There were no persons injured as a result of this event. The elevator was repaired by the elevator contractor and placed back in service at 8PM.
#3 hoist tripped on ""Drive Converter A"" fault. Maintenance foreman and electrician were called out of the mine to check the hoist and reset the fault. There were still two remaining escapeways throughout the incident (#2 and #6 shafts).
A roof fall was discovered by the mine examiner while traveling the return air course along 2C belt line. The location of the roof fall is adjacent to spad 25558. Estimated size of roof fall is 10' high X 20' wide X 20' long.
After loading the holes to be blasted, the blaster went through the final check procedures when one of the loaded holes went off. There were no injuries to anyone and no equipment was damaged.
A roof fall was discovered in the #2 Entry: Inby sapd #250 at the Cook Portal. The fall measured approx. 16' long by 18' wide by 4' high. No injuries resulted from this fall.
Rock fall occurred on 1st left off 3 East Mains at break #13 in the #3 entry on #10 belt. Fall measured approximately 18' wide x 18' long x 8'-10' thick.
Roof fall was found, fall is 100' long x 18' wide x 7' high. Fall is dangered off and stopping lines re-routed around fall area. MSHA investigated the fall area. The roof is laminated it occurred at break 10 on #6 belt on the #1 entry.
An unplanned roof fall was discovered by the mine examiner. The fall location is 5 miles out by the faces on the West Mains. The fall measured 50' long X 18.5' wide X 2-5' high.
An unintentional roof fall was found by the mine examiner. The fall was in the 2nd CC of 5 east main. Fall was approx. 75' long X 18' wide. This was not an active section. Airflow was not disrupted, area has been dangered off. Fall was just outby spad #4188, this is the # 6 entry.
During an inspection of the Hart Street emergency hoist, it was determined that the hoist would not swing away from the shaft after making a trip in and out of the shaft. The investigation revealed an electrical malfunction that prevented the hoist from swinging away from the shaft. Escapeways were re-routed and people notified.
Hoistman lost computer communication to the #2 hoist. A breaker had tripped and had to be reset and the hoist had to be re-synced. The hoist was back in service at 3:05 PM. There were still two remaining escapeways throughout the incident (#3 and #6 shafts).
The mine hoist lost power during high winds. At the time of the power outage, the hoist was empty of men and materials.
Excessive rainfall caused the Tallapoosa river to overflow its banks and enter our pit area. No equipment or persons were involved.

Routine inspection by an outside contractor found the bull gear on the Young mine service hoist to be out of tolerance with respect to its fit onto the main shaft of the service hoist.
During an inspection, a hot spot was discovered by 2 MSHA inspectors & OAEI's plant manager. There were no flames present but steam was rising from a small area on the impoundment. OAEI was not permitted to take any action to remediate the hot spot. Approx 50 minutes later, MSHA issued a ""K"" order. A plan had to be developed before any remediation could begin.
A roof fall has occurred on the 005 Section. The fall measured 50' by 20' and was 5' in thickness. The fall in the number 3 entry at crosscut #72. The area was previously supported with 5' Hy-Tech bolts and superbolts.
#3 hoist tripped on ""East Motor Bearing Overtemp"" fault. Maintenance foreman and electrician were called out of the mine to check the hoist and reset the hoist. There were still two remaining escapeways throughout the incident (#2 and #6 shafts).
Roof fall was in inactive workings in return. Fall measured approx. 200' long X 18' wide X 6-8' high. Roof fall did not impair ventilation or impede travel in any way.
Unintentional roof falls was discovered by MSHA Inspector. (1) Near spad #224 in the return 5500 ft from the face between #7 & #8 entrys, measuring 25 ft long X 18 ft wide X 8 ft high. (2) Near spad #293 in the intake 3600 ft from the face in the #5 entry, measuring 20 ft long X 18 ft wide X 8 ft high. This is to abate citation # 8737577.
A haulage truck caught on fire and the mine was evacuated. The incident was immediately reported to the 1-800 number and MSHA inspector was on site doing a regular inspection at the time. The majority of the fire damage was to the cab of the truck. No fuel or tires caught on fire.
A roof fall occurred at the 2nd Northeast parallel xc-31 #6 entry spad # 21+70 on the supply road. The size of the fall was 25 ft long, 19 ft wide and 6 ft plus in height.
A roof fall occurred in the 2 West 1 Mains, # 4 entry, at crosscut 64. The fall measures approximately 6' thick, 50' long, and 18' wide. The area is supported with 5' fully grouted resin roof bolts.
The Crusher Motor Control Center and Building was extensively damaged by a fire sometime between the night of December 10 and 7:45 a.m. (time of discovery) on the morning of December 11, 2009. The fire was essentially out at the time of discovery. There were no mine personnel on the site at the time of the event.
A roof fall was found on the 1st 54 travelway at appox. 10:50 pm. The fall was 60'x18'x8'. Vent. and passage of men were not affected and no equip. was involved. The area had been mined in Aug. 2005 and supported with 9' cable bolts and 6' tension rebar roof bolts. 8' double-lock bolts were also installed in this area.The fall was caused by an undetected slip in the mine roof.
An unplanned roof fall was discovered in the #1 entry approximately 3000' from the section. The location was at 1st west mains, two crosscuts outby survey spad #298. No equipment or miners were involved. The fall measured 25'x19'x5'.
An unintentional roof fall was discovered in the return course in the #1 entry of the 3rd SW mains of the 002 section. Traveling is not blocked due to multiple entries. The fall was breakered off with cribs & timers. Size of fall was approx 70'l x 18'w x 6'-7' thick.
Elevator car began to vibrate. Machine taken out of service and men brought outside. Machine Repaired.
An unintentional roof fall occurred in the #2 entry of 2 left pillar section. The fall came past the breaker timbers and measured 70'X5""X20'. The area was dangered off and breakered before the fall occurred.
A roof fall above the anchorage of the bolts was discovered in the No. 8 entry one crosscut inby spad location no. 1754. It was found during a weekly airway examination. The fall was approximately 26' long by 20' wide by 4-1/2 feet thick. There were no injures and there was no damage to equipment or disruption of airflow as a result of this occurrence.
At approximately 11:30 Elevator had problems due to brass worm gear in speed reducer failure. Parts were ordered and replaced. Men brought outside.
A roof fall occurred on the No. 3 Section in the No. 3 Entry. The roof fall approximately 15 to 20 feet high For a distance of 30 feet in length and approzimately 20 feet wide. The fall was located inby spad 21325. Additional roof support installed in No. 3 Entry.
#3 hoist tripped on ""Vibration"" fault. Electrician and maintenance foreman came out of the mine and inspected and reset the hoist. Hoist was back in service at 11:15 PM. Hoist tripped two more times due to same fault (11:24 PM and 1:10 AM) and hoist was reset both times. There were still two remaining escapeways.
A roof fall was found in #3 heading at break 17 in the intersection, ventilation was not affected no equipment or persons involved, the fall size was apporox. 20' wide x 9' thick x 30' long. The area has been cribbed and additional 16' rope bolts have been installed.
The mine experienced a partial interruption to power to the plant due to the utility company doing work on the lines off of the mine property. The power feed to the production hoist could not be restored until 10:55 pm. MSHA was notified , and the mine was evacuated at 10:30 pm.
A roof fall occurred approx. 3300 feet outby the E-4 section. The fall was 30' long x 25' wide x 4' high.
The elevator stopped running. An employee was ascending up the elevator. He was stuck in the elevator for more than 30 minutes until they were able to open the doors. He was removed at 11:15p.m. Cause of the interruption was a governor switch grounded out. After the ground was repaired the elevator was run empty, worked properly and was checked by MSHA.
An unplanned fall occurred at break 44 cross cut between #5 & #6 entries. The fall was approx 40'l X 18'w X 7'h. The area had been cribbed, timbered and dangered off at the mouth of the cross cut on both sides of the cross cut.
On December 09, 2009, a fire was discovered in an outcrop area of a coal seam. The fire was not in the coal seam being mined. The fire was not extinguished in required time frame, therefore, the fire was reported.
Roof fall was discovered by th Air Coarse Examiner in Old 4 East x #2 between 1 - 2 room outby 1 xcut & between 2-3 room at the 1st xcut & oubty 1 x cut. Area was bolted with 6.5 bolts 3 piece & 8' cable bolts. The fall was above the bolt ankorage. The area is being secured by cribbing & timber and abandoned.
Roof fall at intersection in the 4 main track entry. Fall measured approx. 17' wide by 17 ' long and about 7' high.
An unintentional highwall fall occurred on 12/8/2009. The fall occurred at Robinson North / Blackberry. No injuries occurred from the fall. The fall severely damaged the #1 HWM Launch and 3 mechanic trucks.
An examiner found a roof fall at x-cut 11, entry number 1, M4 zone. The roof fall was 25' long, 20' wide and 10' high. No miners were hurt or trapped and ventilation was not affected. The area was supported with 6' fully grouted roof bolts prior to the accident.
A roof fall was discovered in the primary escapeway between spad 117 and 98. The fall did not affect persons nor equipment but the escapeway had to be rerouted. The roof fall was 20'x20'x5'high. The area is in the process of being cleaned up.
A roof fall 50' to 60'X 18'wide X 15' to 20' high occurred in the #8 entry between block 58 and 59. At spad # 346 on break inby #336 spad.
A roof fall was found on the 22 East track entry at 49 cross cut. The fall was approx. 30 feet long by 17 feet wide and 2 to 10 feet high. Area was bolted with Install III bolts with resin assisted. The area was screened and we used bacon straps. Fall fell above anchorage point.
THERE WERE TWO ROFF FALLS IN THE 2ND SOUTH MAIN WEST ONE AT X-CUT 82 # 3 entry AND ONE AT X-CUT 82 # 2 ENTRY PROPS ARE BEING SET AROUND THE FALLS AND ADDITIONAL 8FT BOLTS INSTALLED INBY THE FALLS.
Roof conditions of #7 entry outby face 70' had some lamination, and drop in temperature causing the intersection to fall. The intersection was approx. 20' wide by 30' long fell. No injuries and no equipment were involved.
Shonk #1 belt which is an overland conveyor caught on fire at the take up area. No injuries. Caused extensive damage to conveyor system.
On 003 section, a roof fall occurred in the #3 entry - pillar section. All wings on block were mined, ready for butt-cuts. When MRS's were lowered, fall occurred on top and in front of MRS's. Fall approx 35' x 18' W x 8-10' H. Trapped both MRS's. No injuries. Fall reported to MSHA with ""J"" order issued. Modified to ""K"" order at 23:45 hours. Fall was between spad 2503 & spad 2509.
An unplanned roof fall occurred in the Coalburg mine in the #2 entry, 1 brk outby spad #1048 that measured approximately 30' long X 20' wide X 7' high. No injuries were reported. The area was developed on 6/20/09.
Main hoist would not start. Trouble shooting & inspection was done on the hoist. During a start up, it was noticed that M.G. set was making a noise. A shaft bearing on the DC side of the M.G. set was running hotter than normal, and bearing drag likely kept the M.G. set from coming up to speed within the parameters allowed by computer. Bearing was changed out on 12/12/09.
A supply trailer accidentally damaged the outer gate for the man/material hoist at the 11 seam landing. The damage did not render the cage inoperable but it was going to take over 30 minutes to repair the gate so the incident was reported to the MSHA 800 number.

Roof Fall occurred on 1st West Sub-main @ spad 4+90 between 6 & 7 entries. The fall is approximately 20' wide X 20' long X 8' high.
Fire started on two trucks parked outside of our Office/Shop Facility and spread to Office/Shop and burned the facility down. Both trucks burned along with another truck parked inside of the Office/Shop. The Office/Shop Facility was a total loss.
#3 hoist tripped on ""Vibration"" fault. Electrician and maintenance coordinator came out of the mine and inspected and reset the hoist. Hoist was back in service at 3:30 AM. There were still two remaining escapeways.
Roof fall in the #1 Entry of the longwall at Spad 28+33. Fell out between the bolts on the stage loader crushed and blocked part of the primary escapeway. No injuries occured.
Miners working in the 3536 identified a cold joint in the cemented rock fill. They halted work in the area and consulted with supervision and technical services. The recommendation was to use a 3yd jammer to scrape the back and try to bring down the cemented rock fill. The miners scraped the back with the jammer facilitating the fill coming down.
A roof fall occurred on #3 unit, MMU 003-0, #8 entry, at #7 crosscut. The roof fall measures approximately 38' long, 18' wide, and 12' thick. The fall cavity contains a coal rider.
A rock fall was discovered at spad #480 between #1 and #2 entry at 6:45am. It was five to six feet thick and 30 feet long. The area was timbered off and moved outby it. Call was made to Sam Creae at 7:08am and to the hotline at 7:18am. No person or equipment was involved in this occurence.
An unintentional roof fall has occurred between xc 6 and xc 7 of the main west beltline. The fall is approxiamately 18 ft wide, 25 ft long, 6ft in height.
The main hoist would not start. After checking system over, the operating parameters were changed allowing the M.G. Set to start. However, the voltage level was twice the norm. The Static Field Comp, which was believed to have failed, was replaced. The operating parameters were returned to original settings, the hoist operated and was placed back in service.
An extension to a previously reported roof fall dated 7/09/09 in the abandoned C-1 Butt area from the #3 entry approx. 30 ft toward the #2 entry. The fall was 6'-10' high. The area was located approx. 300' outby the face of C-1 Butt & was bolted with 6' fully grouted 3/4"" dia. bolts. The area was cribbed & dangered off.
Roof fall at blk 79 st Lea 1-2 crosscut. No disruption of ventilation or travel. Fall area 15' wide approximately 25' long 7' above coal seam.
Unintentional roof fall that occured in the #1 entry on the HG #3 between x-cuts 9 & 10. The fall measured 25'wx45'lx10'h.
The Caterpillar D10L bulldozer was pushing rock in the shot. As the operator started backing up he turned back around & saw flames coming from the engine compartment on the dozer. Dozer operator got off the dozer but the fire consumed most of the engine.
Fall was found my MSHA Inspector and weekly examiner on the 4th South West Mains between No. 5 and 6 crosscut. Area was dangered off and will not be cleaned up. Fall was approx. 100' long X 12' high X 18' wide.
A brake switch malfunctioned on Koepe mancage. After switch replacement and weight testing, mancage still wasn't stopping at upper level properly. Trouble-shooting continued. Brake pads re-adjusted. No injuries & no other problems.
Two employees exited the 8' shaft and released the skip. They did not secure the skip door in the proper manner. During the next empty trip the door was pulled off after coming into contact with the shaft wall.
Had a roof fall in an old, abounded area of 1 left off 7 east main, area was already timbered and dangered off approx 40' inby spad #7320 in the #5 entry it has a travel way around it and did not effort the air that was circulating this area is to be sealed off as soon as seal plan came back from MSHA. The fall was approx 180' long and 8' to 10' high.
Unintentional roof fall that occurred in the #1 entry of the HG #3 setup room between x-cut 5 & 6. The fall measured 25'w x 45'l x 10'h.
A non-injury roof fall occurred on the active 001 & 002 MMU in the last open crosscut in the #4 intersection. The fall measured 19' wide x 10' thick and 260' long. No persons, equipment, nor ventilation were affected. This area will be rehabilitated.

UNPLANNED ROOF FALL OCCURED ON THE #5A BELT MEASURING 80 FEET IN LENGTH x 20 FEET WIDE x 6-8 FEET HIGH AT X-CUT #31. THE FALL WAS REPORTED TO THE HOT LINE AND ALL ENTRANCES WERE DANGERED OFF. A ROOF FALL CLEAN UP PLAN HAS BEEN SUBMITTED AND APPROVED.
The roof fall was discovered inby the feeder on the C-3 Mains Section, #2 Entry. The fall was 8'x30' and was bolted with 8' SuperTwist bolts. Ventilation or equipment was not affected. No persons were injured as a result of this fall.
A roof fall was found in numbers 1 and 3 entries at cross cut 20 in our M4 zone. The falls were approx. 20' X 20' X 15' high. The falls did not affect ventilation, passage of miners or cause injury. The falls occurred in the intersections only. The area had been supported with 6' fully grouted roof bolts. The MSHA hotline was notified of the accident.
The roof bolter failed to install roof bolts properly along with bad ground conditions which contributed to this roof fall. This caused an area 30'X 40' X 12' deep to fall.The roof will have to be repaired or abandoned.
An unplanned roof fall occurred at spad #4317 on the 001 section between the #3 and # 4 entries. The fall occurred approx. 2400' from active face. This area was developed approx. 10/15/09. Fall measured 30'L X 20'W x 5' H. Ventilation and passage of person was not affected. Area will be rehab'd. State and Federal Agencies were notified immediately.
Pre-shift examiner was preshifting mine for Tues AM in 4 West Section. He stopped at pump along Intake in #7 Entry, he smelled fresh resin. He went ouby 5 xcut in #7 Entry where he found a roof fall in the Inter. 4 xcut outby STA at 8152. This had been bolted with 4.5 2 piece bolts & 8' cable bolts. Area is cribbed off & abandoned. Fall measured approx 35' long 70""x 8"" high in Int.
The outby mechanic was doing his permissibility on LC #8, when he went to location he found a roof on top of the 7200 cable. He went & got foreman & knocked power. Area approx. 100' long from STA #8302. Area was bolted with 4.5 1 piece bolt. The area has been cribbed off and 7200 re-routed. This fall was in South Main Section. The height at the fall was approx 5'.
Employee was operating a remote control continuous miner. He was struck by a rock. He did not survive the injuries.
On December 12, 2009 at approximately 6:38 am, a pre-dawn, low temperature accident occurred on a haulage road at PE Southern Pike Co. The driver of DT 178 exited the loaded coal truck, resulting in his death.
Miner was walking along the longwall face when he slipped and his foot went between the relay bar and toe of the shield and was pinched. This involved fractures of the toes on his left foot a nd also the loss of skin on the first two toes of the foot. The bones of the first two toes were still attached but toes of foot were surgically removed after accident.
Employee was atempting to clean the dumping point with a scoop. As he passed underneath a low hanging cable he attempted to raise it up causing his left little finger to be mashed between the scoop canopy and the mine roof. This injury required stitches to be installed.
Employee was helping clear dump chute. Rock came loose, mashing right thumb. Partial amputation to first joint.
EE was installing a roof bolt he was pushing bolt into the hole with machine. When roof bolt plate was only inches from roof he noticed that it was misaligned for curtain hook and reached up to swat plate to rotate it. His glove caught on plate, tip of his right index finger between plate and mine roof. Approx 1/8 to 1/4 of his finger tip was amputated.
The EE was pushing a hand truck into the Mill room thru the center overhead door and stepped on a mill ball with his left foot. As he stepped on the mill ball he felt a sharp pain in his left knee, as he felt the pain in his knee he shifted his weight to his left side. Note well: the diagnosis of a hernia was made on 01/21/2010.
While cleaning out a chute from a screw conveyor to an elevator, with a metal bar, the employee suffered a laceration to the ring finger of his right hand.
The drill crew were in the process of securing the drill to the face. As they were moving the foot plate to the face plate, driller placed his hand in the pinch point between the face plate and dui-dag. Right Middle and ring finger pinched. Stitches to tip of right middle finger and amputation of ring finger tip (distal).
Due to severe weather conditions the scalehouse to the Quarry was closing. The employee had removed the trash and was walking back to the scalehouse when he fell in the parking area. An ambulance was called and the employee was taken to the hospital.
The employee stated that he was traveling behind the #7 feeder and he stopped to pick up his water bottle that had fallen. At this point he states that a scale fell and struck him on his hard hat. The foreman could not locate a scale. He then took him back to find the exact location. He stated he was not injured just a little shook up.
Loose rail cars collided with the locomotive causing the operator to strike his hip on the handrail. *DID NOT START LOSING TIME UNTIL 1/26/2010
On 12/31/09, EE dropped a 2"" pivot line off the end of a barge, the weight of the line pulled his left shoulder, causing pain. EE returned to work the next scheduled shift, but began missing work on 1/7/10, McElroy was informed EE was off work due to a work related injury, via a WC-1 on 1/14/10. Now determined to be a reportable incident.
EE working in a chute - removing scaffold supports. Employee was stabilizing self from rigging above his foot slipped, employee caught self with right arm strained shoulder
EE slipped on the ice and fell and bruised his right shoulder.
Employee states that he was looking up holding carbon with rope and stepped into a muddy hole causing injury on 12/31/2009, due to time of injury and holiday I was not informed of injury until 01/04/2010. Employee was restricted from work 12/31/2009-01/04/2010.
EE was on job of opening up kiln hood all day--entailed wrenching or burning off bolts. Near end of day EEs R elbow started hurting-in looking was red/swollen. EE went home, saw something & dug it out w/tweezers--EE alleges little metal sliver Returned to work 1/4/10, reported pain in elbow to super and requested med treatment.
Operater of a 980F Cat loader was checking a possible problem with his loader. He slipped and fell off of loader, breaking his left ankle.
Employee was walking from the 101 packer area to the restroom. When he stepped down from the elevated area by the bulk elevator he tripped on an empty sample tray that had been left on the floor. He fell and hit his left forearm on the concrete ledge.
Employee was installing rib bolts in crosscut when he stepped from the platform he slipped and turned his left ankle.
Injury while helping repair a drill rig, received burns to face and back. While he and mechanic were using a torch to break loose a sub, oil inside the sub caught on fire without their knowledge. The fire blew out causing burns to his face and body.
EE entered prep plant - walking on smooth walkway and rolled left ankle
Employee was reaching to open a ball valve & experienced sharp pain in his left shoulder before operating the valve. He reported to medical, was treated & returned to work with no restrictions. Employee continued to complain of pain. An MRI revealed tear in rotator cuff. He was referred to a specialist & was scheduled for rotator cuff surgery on 2/3/10.
Member was working to unstop a drain pipe that ran from second floor of plant to the basement. He entered a restricted area and was walking across a metal sub floor to reach the pipe. A section of floor broke and he fell 15' 9"", striking a Catflock tank and sliding down it to the basement floor.
using trackhoe bucket to lift cylinder. Operator or trackhoe lowered bucket too low and struck employee in head.
Employee was closing the manually operated equipment doors located in the 3 North 1 Mains. Upon closing the first door, the second door closed on his left forearm. The employee sustained a fractured left forearm.
EE stated that while cleaning up around No 4 belt head he slipped on a rock and fell. When he got up he could not put much weight on his foot.
Employee was putting a breaker in at No. 6 belt head starter box when the breaker flashed causing a burn to his hand.
EE was throwing salt into the bed of his trailer. EE was inside of salt building getting salt out of the barrel, turned to throw the salt and stepped on loose salt on the floor. Foot slipped and EE tore a muscle in his lower back.
It was snowing with slippery conditions; the employee fell and strained his groin.
EE fell while in his office in the Engineering Bldg. During the fall he struck the back of his head on the edge of a table. EE received a laceration to the back of his head and indirectly experienced a subdural hematoma to the right frontal lobe.
Employee was hanging miner cable in 4B when he felt a pop and pain in his right shoulder. He continued working and did not miss any work until May 5, 2010 when he went off for surgery for a partially torn rotator off.
At around 8:45am, EE met with plant manager at the office. The plant manager noticed the EE's speech was slurred. The EE said he felt sick and needed to go home. Given his health history with heart problems, the manager had him taken to hospital. At 11:15am, we were notified by hospital EE had possible heart attack while undergoing test. Later notified was not a heart attack.
Employee stepped off the rear end of the Fletcher track roof bolter and apparently twisted/injured his right knee. He continued working and his last day worked was 12/30/09.
EE was assisting in making a belt move when the tailpiece kicked sideways. The side board knocked him into the rib, cutting his head and mashing his left thumb.
Employee was returning to work area after dropping off tools. He said his feet slipped out from under him and he fell. He tried to catch himself with his right arm and shoulder and fell his shoulder. Employee will be getting an MRI on Friday but at this time pain is his only symptom.
Employee walking down steps in Mill and stumbled on step, falling on right elbow.
Employee was removing a bolt with an impact wrench. When the bolt spun without loosening, he reached into the engine compartment to find out why the bolt wasn't coming loose. Sharp edge, out of sight in compartment then sliced through his glove cutting his right index finger.
While leaving a work area the employee tripped while going down a flight of stairs and bumped and cut his head. Employee was treated at ER and release the same day.
On 12-27-2009 a employee was pulling the cable off of the roof bolter reel when he felt a pain in his right shoulder. He continued to work but a dull pain continued. He started missing work April 01, 2010 when he was diagnosed with muscle damage that required surgery.
EE was operating shuttle car and hit a bump. EE bumped against inside of cab bruising L ribs and shoulder. EE was treated and released to perform normal work duties. Subsequent medical evaluation generated restrictions and resulted in lost-time 5/5/10.
EE was cleaning up from replacing a hose clamp. As he put the hose clamp into the truck, a burr on the clamp caused a laceration to the left palm, requiring 5 stitches. This report was submitted online on 1/4/10 at 3:30pmPST but did not go through. MSHA help desk was informed and this is the second attempt.
Employee tripped on a small gravel pile and fell, resulting in a fractured bone in his left hand.
Attempting to start pump located behind splitter box. When he attempted to walk around the splitter box through approximately 8 inches of water, his left knee gave out. Marfork Safety Department was informed employee returned to work but he called today 2/1/10 and reported he is scheduled for surgery 2/17/10.
It was snowing and EE stepped on a slight depression on the ground; he slipped and fell resulting in a fracture to his right fibula.
Ground gave way. EE was caught between concrete and ring steel. EE received a fractured tibia.
On 12/28/2009 we were notified that the injured worker was breaking ice with a chisel and hammer. The injury was caused by a repetitive motion.
EE was walking on unpaved snow covered road to clinker silo for sample when he stepped into a hole and twisted his left knee. EE was able to perform regular job duties unitl surgery which is scheduled on 02/09/2010.
Checking battery cables when battery cover door fell on left hand, smashing & cut left middle finger.
Large fall behind longwall shields caused air concussion that pushed employee into coal rib of #3 entry (headgate). Injury to employee includes cut on top of head, broken ribs, broken scapula. Employee was transported to hospital for treatment of injuries.
Employee was lifting a sprocket with come-a-long at the de-water, when the come-a-long fell, hitting the employee on his left hand against another chain.
Employee was stepping down from tractor on his right leg. Employee felt pain in lower back. Employee did not start missing time from work until 01/04/2010.
The employee was straightening a hydrate bag that had jammed when it opened and blew lime into the employee's right eye. Has received medicated eye drops and missed one day of work.
EE strained his neck when he trammed a shuttle car up onto the boom and when he backed off was jarred.
He was attempting to pull a drill steel from the roof and felt pain in his left shoulder. He continued to work and have pain. He was eventually removed from work for a re-tear of the supraspinatis on 1/26/10. EE was off work beginning 5/17/08 for the same injury. Please consider this incident Occupational Illness due to repeat trauma as indicated by MRI report.
A rock fell from the roof between the roof bolts in the #4 entry of the 1st E panel striking the employee in the back. This caused him to fall and twist his left ankle. He went to the Dr. during our Christmas vacation and is unable to return to work as scheduled on 1/4/10. Due to the ankle injury.
employee was helping to clean out a 3"" line used to pump concrete, pressure built up in the line and when material came out, the end of the pipe kicked backward and hit EE in the right leg.
Climbed up on 777 Truck to check the oil and reported feeling a stretching sensation in his left knee when he began to kneel down to open the engine acess compartment door. He stated he did not slip, strain, or fall. He worked until 1/29/10 when he had arthroscopy w/menisectomy on his left knee. EE began missing work on 1/29/10 and became lost time reportable accident on this date.
Employee slipped and snow and injured left shoulder.
EE was walking from his pickup truck to his road tractor when he slipped and fell on ice in the parking lot behind the Unimum Crystal Plant at the loading dock. He was taken to the emergency room at Blue Ridge Hospital. The ice was from the 12 plus inches of snow & ice received on 12/18/09 & 12/19/09.
Each of 4 miners on duty confirmed conditions were dry and did not contribute to the accident. Eq. was in proper order at the time of the accident. EE was climbing onto the machine to do a morning pre-op, lost his balance and fell off of the machine. EE went to the emergency room, the doctor did a CT scan which was negative. However the EE fractured both wrists.
EE said he was positioning a chain break on the conveyor chain of company #46 shuttle car to separate the chain to repair it. While placing the chain break on chain, it became wedged on chain. Injured ee placed his hands under it to remove and reposition it. Chain break fell approx 3"" & struck hand, resulting in a fracture to the left ring finger.
Employee was measuring a timber when the rib rolled stricking him from behind and knocking him down.
Employee was roof bolting when the drill steel got stuck in the roof. Then the steel fell out of the roof striking him on the right hand little finger. He received 17 stitches on his finger.
EE was building a crib and was picking up a crib block and twisted around and felt pain in his lower back.
EE had just put a ladder in back of a pick up truck as he was walking to the drivers door he slipped on the ice and fell. A light dusting of snow had fallen and covered the ice.
EE was troubleshooting an electrical issue with compressor, removed access panel to access relay inside cabinet. Upon job completion, EE reinstalled access panel and felt a tear in his right shoulder. On 12/21/09 EE was breaking loose a large icicle from air cannon, thus aggravating shoulder. EE was able to perform regular job duties and surgery is scheduled for 1/15/10.
Rock (2'wide x 2-3"" thick) fell from top between bolts and struck employee on left shoulder.
Employee jumped out of the bed of the company pick up truck near the mine office at the completion of his shift when he experienced pain in right knee. Employee continued working until January 7, 2010 at which time he started missing work making this accident reportable.
Operator was exiting the drill unit cab and turned his right ankle. Operator was taken to Emergency Room due to pain and swelling of ankle.
Climbing down steps, foot slipped & sprained right ankle.
EE was carrying aluminum monorail along the side of the tailpiece and slipped. The monorail slipped from his hands, fell and hit his right knee cap. The EE finished his shift, but decided to see a Dr. at a later date. The 2 MRI images of the right knee showed degenerative joint disease with no tear or internal derangement. Please consider this OI. MRI information attached.
While pulling the fuel hose down off a machine, the hose caught left wrist causing a sprain.
He was kneeling in the mouth of # 5 entry, by the curtain on # 2 unit when the scoop come through the curtain striking injury with the bucket.
EE trammed the dual head roof bolting machine from No. 6 heading to No. 5 heading. He backed the machine up into No. 5 heading to turn around & take bolter to No. 1 heading. As he was backing up the bolter-cable fell off the back of the bumper. EE attempted to put the cable back on the bumper with the bolter still moving & caught his hand between bolter & coal lying on rib.
Individual was crawling to the head when he crawled between 4&5 shield a rock fell between the shields and struck the injured on the leg.
Stepping off crusher and slipped. Twisted ankle returned home felt ok over weekend never mentioned incident. Returned to work 12/21/09 stepped off excavator track and hurt same ankle, went to Dr. on 12/22/09 sprained, doctor recommended 1 day rest and or brace.
EE was switching hoses on the NaHS railcar. When he disconnected one of the hoses, the hose blew and splashed liquid onto employee and into his right eye. He was wearing all the required PPE.
Worker was stacking 50# bag, failed to walk around pallet to stack bags, instead walked over pallet. Possible sprain right knee. Worker sent to doctor.
Employee was helping to set up concrete forms. He went to step over a stationary form already in place and misstepped and tripped over a form. A co-worker was trying to put in place but was not yet stationary and EE lost his balance & fell on left knee on frozen ground. No lost time - no restrictions - will do physical therapy.
The accident involved a truck driver, who is employed by Hecla Machinery & Equipment Co. The truck driver was on site to pick up a load of coal. According to the loader operator who did not see the actual fall, the truck driver fell on the ground hitting his head on the truck step. First aid was administered and 911 called.
EE was cleaning the tracks on his cat hoe, when he stepped on a rock and fell, landing on a rock. He cracked a rib.
Employee was descending a stairway and his Fall Protection Equipment (Lanyard) caught a Handrail Post. This caused the employee to twist his right knee and fall to the floor level. Employee was later sent to a doctor, and the diagnosis was cartlidge and ligament damage which will require future surgury.
Employee fell while watering roadways at the 9 South Mains section. He didn't miss any work days until 1-6-10. No diagnosis was reported until 1-20-10.
Employee was coming off the plant elevator and hung left foot on elevator door causing him to fall on right arm and elbow.
EE was bolting top, he had put extension on the steel and was raising it into the hole when the steel pressurized in the hole and it sprung into the drill chuck hitting his hand.
On 12/16/2009 EE as he was setting a timber and the rib sloughed off and knocked the timber and EE to the ground. As EE reached towards the round while falling, he injured his right shoulder. EE worked untill he had surgery on 4/1/2010.
Driving haul truck, being loaded by an excavator, a rock fell off the bucket as it moved over the truck, hitting the left rear corner of the box causing the operator to spring forward and then back in the cab, possible whiplash.
EE states that while he was hanging mine phone at the old ""C"" section power center, as he was attempting to climb up on the power center his left knee hyper extended. EE finished his shift. Accident was not reported to mgmt. until 12/17/09. EE did not notify mgmt. of medical findings until 12/31/09.
Employee arrived at the shovel to start the PM. As he was walking to the shovel he slipped on some small rocks falling to the ground. Employee was taken to the safety building to be checked by a First Responder the decision was made to take the employee into the ER. Employee was taken off work.
Installing 10 cable bolt; placed bolt wrench in chuck, stepped back to reach cable bolt. Stepped on a rock & strained right ankle.
The man was striking a pipe with a sledge hammer when the hammer deflected and struck the man in the ribs.
EE claims that on December 16, 2009 he started having a numbness sensation on his legs. On December 17th he went to the emergency room to receive treatment. The employee claims that he is still very symptomatic and on December 21st he went to CFSE to receive treatment.
EE was hanging J-Channel at 62 xc 5-6 with a co-worker. They had the J - Channel on shoulder lifting it to roof to strap the J - Channel off. EE slipped on uneven ground falling to his knees. He continued working until lunch time, he sat down for lunch, when he got up after lunch he noticed his right knee was sore.
Shoveling tail piece lift shovel of coal and felt burning in groin area.
Three employees had hooked up a battery charger to the batteries of a 988B endloader. Two employees were on the opposite side of the loader when they heard a noise. They went to the other side and found EE laying on the ground. They did not see what happened. EE doesn't remember anything about the incident or what he was doing.
An employee was jostled when his loader contacted a toe at the shovel site. He was evaluated by a Physician & taken off from work. It was made known on December 18th that it was a lost time injury.
Employee was fueling a dozer. He slipped on frozen ground and strained/twisted his lower right leg.
Employee complained of pain in his shoulder after lifting a wheel motor cover off of a haul truck. He was evaluated by a physician and had surgery on 6/22/10, resulting in lost time.
Employee stated he was helping hang the Miner cable and his left shoulder started hurting. This shoulder has been bothering him for a while, but this time it seemed like it was worse. He continued to work until he was taken off by the doctor on 02/22/2010. Imsending the operative report to show that this was a illness. They had to trim a bone and spur off.
EE was operating the shuttle car traveling through the #3 CC. The operator hit a bump and the operator bounced out of his seat hitting the top of his head against the canopy. When the EE fell back into his seat the EE injured his back and lower legs. The EE did not go to the hospital until late 12/18/09. No broken bones found possible bruise and strains.
Employee was operating manbus and rocks fell and struck him on the head EMPLOYEE DID NOT START LOSING TIME UNTIL 4/5/2010 DUE TO SURGERY.
Employee stated he was changing transmission in a Dapco three wheel rib runner. While lifting up on the transmission, employee stated he felt a sharp pain in his back. Started missing work on 12/16/09.
He was making a weekly exam in the bleeder when he slipped and fell causing a strain to his lower back and left groin.
Employee lifted 148# bucket cover from the ground to bumper of service truck, causing lower back strain.
Employee was exiting the warehouse at the end of shift out the Northwest man door and down the steps. It is unkown if she had her hand on the handrail. When she took her last step to the ground her foot hit the seam edge of the asphalt surface which drops about 1"""" and rolled her left ankle.
He was attempting to go through a high pressure regulator when he lost his footing and fell, causing a fracture to the left lower leg.
Ee is undergoing surgery on this date for an umbilical hernia. The hernia occurred October 21st. EE received NO medical treatment and NO work restrictions. The employee returned to work the same day as the hernia occurred, and has been working ever since, until 12/14. On 12/15, the employee was laid off due to lack of production, and is having surgery.
Employee backed onto 3/4 rs pile to dump, pile was undercut and caved, flipped truck over once landing on roof.
Tramming the continuous miner and placing himself too close to the miner and the coal rib, pinching himself between the body of the miner and the coal rib, breaking his left leg and bruising the right leg.
Employee was exiting cab of locomotive and while stepping out on the two steps that stepped down to the cat walk he did not get solid contact with the 2nd step and twisted his leg. He grabbed the rail and stopped his fall but his leg hit the catwalk with alot of force...breaking the bone below the knee in 2 places.
An employee was operating a haul truck and began experiencing lower back pain. The employee was being seen by a doctor and on 12/14/2009 recieved a no work release for one month.
Employee was walking across driveway to the break trailer when he slipped on ice and fell landing on his right shoulder. Restricted duty assigned. **Employee did not report the injury until 1 2/15/2009**
Was pulling water pipe and felt a pain in his back
Welders were replacing a section of flooring on the catwalk on the east side of the 5X14 screen and had cut an approximate 1' diameter hole. EE stepped onto the hole and lost his balance and fell. He struck his right shoulder on the handrail or screen, fracturing arm bone.
EE was being lifted on a platform with a forklift to loosen cement stuck in Portland screw. When forklift driver lowered the lift, the platform jerked and the EE lost his balance and fell to his knees. Has pain in his knees
Employee was walking across the quarry surface, he slipped on ice and fell on his left shoulder resulting in a contusion of the left shoulder.

The employee was unloading track rails when some of the rails fell off the car and struck his right leg.

The employee was opening a lid on a tanker truck to begin filling it with CKD and the lid blew open due to pressure build up blowing the dust into his eyes, burning them.
Using the Grader to plow snow from the quarry haul road. He felt a loss of hydraulic pressure in the machine. The operator stopped the machine immediately to investigate issue. As he stood up in the cab and turned around the employee stated his foot got hung up on the cab door threshold causing him to fall out of the machine onto the ground (landing on knees/legs).
While Dry Fork Mine does not believe this incident constitutes an accident under MSHA's regulations or the Mine Act, EE was applying heat and compressed air to a partially frozen water pipe when his leg was injured.
Employee was attempting to light a torch and it flashed back and he sustained burns to his face, ear, neck, part of his arms and part of his hands.
Managment was notified by the employee on 12/29/09 that he sustained an injury to the lower back on 12/13/09 as he was shoveling frozen dirt to allow for additional room to install a belly pan on a dozer.
Employee reported that he was clearing a jam on top of sand plant when his foot slipped and he fell. He reported that he hit his head, right shoulder and right jaw. He reported throbbing in his head. Blurred vision and speech was slightly sulrred.
Original injury of 12/13/09 was not MSHA reportable. EE states he lifted 24"" pipe and heard ""pop"" sound. No medical treatment until 1/18/10. Physician took EE off work 1/19/10-2/3/10 for low back strain. Full release to work 2/4/10.
Not enough slack was pulled for the buggy car head on the previous shift. EE was pulling the cat head while another worker was feeding him slack. The cable ran out of slack and jerked him down.
Employee had finished his shift and was removing his boots in the bath house when he felt a pull in his left calf.
Employee stated that he hit his right knee on the side of a bridge conveyor while attempting to cross it.

EE was operating the longwall shearer. While walking by the shearer a piece of rock approx. 10""w X 30""l X 10""t came out of the pan line and struck EE on his right foot.
Employee was reaching to pull drill steel out from rib and his belt hit feed control which let the drill pot down on his right foot.
He was lifting a flyte while standing inside a feeder, feet slipped and he fell over the side of the feeder, landing on his back. This employee continued to work normal duties until 5/10/10, at which time he was takken off work when it was discovered he had back sprain relating to the injury.
EE was waiting for the haulage unit to back-up so he could secure the bolter cable to the mine roof. EE thought the MBC unit had backed up past him when he stepped forward to begin hanging cable the operator moved to adjust the MBC trapping EE's left foot under the cat.
Employee reported to medical dept on 12/11/2009 stating soreness in shoulder. Treated conservatively. Continued working without meds or work restrictions. Diagnosed with torn rotator cuff and scheduled for surgery 1/6/2010. LWD started 1/5/2010.

Stepped on floor grate in water plant. Grate was out of place and it gave way causing he to fall through opening. He caught himself on floor buy soffered bruises, strains and a back sprain.
Employee alleges he was climbing down ladder and he missed his step and twisted his right foot causing a sprain to his ankle.
EE was working to construct an overcast. He had stood up a metal leg and while attempting to place a bolt in the leg it fell over and struck him on his lower leg.
Employee was injured attempting to turn on baghouse control switches for dryer fan motor. Control panel exploded hitting employee in face and chest.
Employee was changing a fuel filter on a CAT D10 dozer, when his foot slipped on the track, causing him to fall to the ground on his left arm. Jammed his arm around the elbow.

EE had helped setting the MRS's up in the No 5 entry when the right rib rolled off striking him in the right leg.
This accident was reported in Dec.09 as a minor incident.EE was servicing a truck bumped his head he refused medical treatment continued to work and on Feb. 25,2010 we were made aware that the EE had taken off work due to the 2009 incident.EE is now under Dr. care.
DID NOT START LOSING TIME UNTIL 12/21/09. Stepped off roof bolter and twisted his right knee. Surgery required.
Employee stated that he bent over to stencil a bag, then felt a pain in his lower back. Employee had trouble straightening up. Employee reported incident at 7:45 a.m. and stated he felt better moving around, then returned to work. Employee was sent to hospital for evaluation at about 9:45 a.m.
When ee was mounting a transmission of a truck (7738) he felt a strong pain in the back and he could not continue with his work.

Injured had lifted a roller into place at unit #119 dryer at Pinnacle Prep Plant. Injured worked until Jan 15, 2010 and was off work until Feb 12, 2010. Injured stated that his back was hurting prior to Dec 9, 2009 and that on Jan 15, 2010 he had lifted a 5 gallon bucket of anti freeze feeling a twinge in his back went to see a doctor. MRI showed degenerative arthritis.
Employee finished refueling equipment spread & was rolling up the fuel hose in the rear of grease truck. A wind gust caught the rear door of the truck slamming it into employee's head knocking him down. The door was fastened at the time of accident/injury, per accident witnes. Weather was very windy, with gusts ranging upwards up to 40-50+ MPH.
Employee was climbing onto a D11 Dozer. He was stepping over from push arm to tracks and slipped and fell onto tracks. Went for xrays. Had a bruise on his rib cage.
EE was pushing coal at loadout on #12 pile feeder & upon backing up the dozer dropped into a hole (appx 4') created by push & position changes of dozer. He was transported to hospital after feeling pain in his lower back from the jar. He was diagnosed with bulging discs, referred to a specialist for eval. on 12-23-09. Diagnosis: pulled muscles & strained ligaments. Therapy
At the end of his shift miner was walking to his vehicle, stepped wrong, turned and felt his right knee pop.
EE was shoveling sand near the BC13 head pulley for one hour to clean up the area and allegedly felt pain in his left shoulder. The X-ray was negative and he continued to do his normal job. He had a medical procedure on 02-2-2010 that made it reportable to MSHA. He continued to work. He had surgery on 3/8/10 that changed it to lost time status.
Employee was standing at roof bolting machine when scoop backed out of right break into #4 entry. Bucket of scoop caught corner of rib and broke rib loose causing rib to roll onto EE and roof bolting machine. Employee received a fractured collar bone and 6 left ribs were fractured, employee became lost time on 12-10-09.
EE was watching the miner cable when a piece of rock fell from the rib/roof and struck the EE on the shoulder.
EE was checking oil in truck and slipped and fell backward caught right leg in bumper on the truck. EE fell backward had to have help on getting back upright.

While servicing Komatsu 475 bulldozer, EE fell from top of dozer tracks, striking his back on the ground.
Employee picked up a roll of shrink wrap. The shrink wrap started to fall out of his hands, the employee tried to catch the roll of shrink wrap and felt a sharp pain in his lower back.
Employee was side bolting in #3 entry, F-19 Section when a piece of rock measuring 24"" x 47"" x 1/2"" to 4"" thick fell and bounced off of the ventilation tube striking him on the head resulting in a fracture to the C-5 vertebrae of the neck.

Injured started to walk between a rib and a coal hauler when the rib rolled and pinned his left leg against a regulator that was loaded on the hauler. Injured received a compound fracture to his left tibula.
While working on the 12 I longwall face, the employee got his left foot caught between the panline and a toe of a shield.
EE stated that while operating the excavator, a rock fell from the bucket causing the boom to hit the ground, jarring him.
Injured was picking up a bundle of PoGo sticks. Approx 50 to 60 lbs. Ruptured lower stomach
An Industrial Designer, employed by Tegron Industrial Automation, apparently opened an MCC disconnect and alleges to have received an arc flash burn while measuring for a control transformer. He apparently sealed the enclosure back and left the plant without notifying anyone of his injury. Tegron contacted the plant on Monday, December 7, 2009 and reported the injury.
EE was standing beside of the roof bolter when a piece of draw rock hit him on the left shoulder
Employee had thrown cable over out of the way and when he turned he walked into a bit bucket bolted to the roof.
EE was driving a personnel vehicle when he lost the brakes and ran into the back of a ram car.
Employee was swinging a sledgehammer to drive in a bushing on a continuous miner. He had pain in his shoulder that persisted. On 2/3/10 he reported to the operator that he was under doctors care. He had not missed any work days or had any restricted duty at that point. On 2/4/10 he began to miss work making this injury reportable as a lost time injury for December of 2009.
injured and coworkers were recovering old belt take up unit when a beam struck injured in the head. this resulted in a concussion and stitches being required.
Miner was performing preshift inspection. While walking around truck he slipped, falling onto his right ankle and foot. Resulting in broken foot.

Climbing down steps and thinking he was on the bottom step when, in fact, he was on the second step three feet above the ground, he let go of side-rails and fell on left hip on hard frozen ground - breaking the hip as determined at the hospital that he was ambulanced to.

EE loaded his tool box on jeep and felt back pain. Man was seen at Occ Health a few days later and has worked restricted duty since. He is scheduled for an MRI this week, 1/4/10. Accident not reported till 12/7/09.
EE was standing on a ladder about 2' off the ground, the ladder slipped on the icy dredge deck and the EE caught himself with his right arm straining that side of his body. Worked 12/04, missed work on 12/05. EE did not see a doctor until 3-4 days later.

The employee filed an alleged report stating he felt a pain in his shoulder while wiping a windshield. He was evaluated by a Physician and it was made known on January 5, 2010 that Phyical therapy had been started.
He was walking from the belt to the track entry at 19 rm north east when he stepped on a piece of coal and twisted his right ankle. He continued to work at his regular duties and continued to have pain. He was eventually removed from work for possible ligament tear of the right ankle on 3/2/10

Employee was working on a seperator in the plant when he got his left hand caught in a pinch point under the seperator resulting in one mashed finger and one fractured finger.

EE was preparing to exit the steps at the back of the launch vehicle to do a welding task on a hydraulic jack end. When his right foot contacted the frozen ground he twisted his right ankle, which has been diagnosed as a sprain.

Employee was burning with a torch. He was not wearing a welding jacket or apron. He was bent over a job when the oxygen line broke and saturated two layers of clothing and cutting sparks or flame ignited, causing second degree burns on abdomen and neck.
He and another employee were dragging & pulling a mini-conveyor from the belt line to a truck, employee felt strain in groin left side. They received assistance from 4 others to finish moving conveyor and load in truck.
Crusher with inadequate dust control was moved next to control shack, subjecting operator to high levels of rock dust. This resulted in shortness of breath and sore chest/lungs. This condition (excessive dust) persists. I am seeking medical attention today. I have missed three days of work so far. Time/date is approximate as condition grew worse over several weeks.
Employee states that he was stepping out of the cab of the shuttlecar when he caught his foot on something inside the cab and fell, twisting his knee. Lost time reportable injury March 1, 2010 due to surgery.

While removing a roll of high pressure hose off the duck bill of the scoop, another roll of hose rolled off the scoop duck bill, striking EE on the left knee. EE continued to work with no restrictions until 3/12/10, when he went off work for surgery to the left knee.
Injured employee was adjusting water on a cone crusher and stepped on side of concrete pad that supports the bottom of steps with right foot, resulting in a sprain.
Employee was operating motor grader, other employee was approaching in haul truck, haul truck operator hit brake and started to skid, when mg operator turned. Haul truck operator hit mg.
Removing coal ring from kiln when strained neck and shoulder occured.
2 employees were attempting to close a roll-up door. One employee was pulling on the door and a 2nd employee was waking to the other end. As the 1st employee added more tension, the door fell to the floor. The 1st employee tried to get out the the way but the door scraped his back and he fell to the floor with his lower legs pinned under the door.
While Employee was climbing up the stairs on a volvo haul truck, he lost his footing and fell to the ground.

At truck scales and opened load cell for cleaning. Pulling fire hose and stepped into the hole he just opened. Stated he was fine. On 12-23-09 came to us and stated he is experiencing pain so he was sent to doctor. Diagnosis ankle sprain. Gave him ankle brace and restrictions.
Employee was pulling on a piece of belt using channel locks. The channel locks slipped off and he fell on his back. He started having pain in back and left leg. He started missing work on 2/24/10 to have surgery.
Continuous miner operator was tramming the miner from place to place when the miner left track started to spin and kicked the conveyor boom against the miner operator. The operator received 3 broken ribs
While cleaning mud out of the shaker screen the employee twisted his left knee. The employee had surgery on 3-22-10. This accident did not become Lost Time until 3-22-10.
While lifting a pump with a backhoe, an aluminum hose fitting broke, causing the hose end to swing out and strike the employee in the forehead.
Ice was covered with snow and employee didn't see ice. Employee slipped on ice injuried right arm & shoulder. Employee was hurt on 12/11/09 and reported incident to management that day, at this time employee did not feel he needed to see doctor. On 1/4/10 reported pain in his shoulder, was sent to clinic and was put on restricted duty along with prescription medicine.
Employee was working in the hell hole of a D-11 caterpillar dozer. To remove a breather from the transmission. When the breather broke loose from the transmission. It caused the employees right arm to move up and back striking a fire suppersion nozzle. Causing a fracture to the right elbow.
Accident was first reported to Safety Manager, on Jan. 4, 2010, at 8:45 am. One of the employee's job duties is Loader Operator which requires him to pull the wheel chocks used to secure the loader while it is parked. While performing this duty, he felt a pain in his lower back but continued to work. EE is on restricted light duty, with no lost time.
Stone slipped while placing stone into rubbish box. Cut went through the glove and cut ring finger on left hand.
On November 23, 2009, employee strained his back while performing work activities in the plant. Employee felt pain in his lower back after shoveling, lifting a rock out of the roadway, and pulling a water hose. Employee did not request to seek medical attention until 12/9/09. This accident is being reported on 12/16/09 which is within 10 days of seeking medical attention.
An operator slipped when descending stairs, and as his right foot slipped it caused his left leg to enter the handrail. After his left leg went through the handrail, it caused him to spin around and fall the remaining 3 steps hitting the ground, landing on his back. Operator got up and felt a strain in his left knee.
While mopping a floor, employee felt a pop in the left side of her lower back. Note: Employee was permanently transferred 3/1/10. Her transfer was due to seniority at the Co. Safety was not made aware of the transfer until April 20th.
Employee mounted his dump truck without using the handrail. He reached for the steering wheel to pull himself in. He missed the steering wheel, falling from the elevated position down to the ground. He landed on his rear braced for impact with his left hand. Employee suffered a sprained wrist on his left arm.

Employee was attempting to remove an air hose from cyclone at roaster to install it on another cyclone. When removing the hose, the isolation valve came off and hot fine ore blew out and hit employees hand and thumb.
Employee opened engine access door during inspection and pulled lower back.
The person was attaching rope that held cable to the miner tail with the miner in motion. No one shall be in pinch points with any equipment running. The person should have waited for the miner to stop and turned off before attaching rope.
EE twisted his back while going up stairs aggravating previous back injury which occured on 5/10/2004.
Employee and mechanic were thawing out an air line when it ruptured. Alcohol in the line flashed and employee was burned on his hand.
Driver was driving tandem coal truck from underground mine to prep plant on the normal haul road, when he got too close to the ditch in a curve and started spinning in the mud on the side of the road and slid into the ditch, causing the truck to lay on its side. The driver hit his head on something inside the cab, giving it a small cut.
Emplyee hit his thumb on the pinner steel while bolting and broke it in 3 places.
EE was climbing scaffold to tie tarp to scaffold to reduce wind while welding bed on CAT 793 dump bed. Keeper for pin on scaffold crossbar was missing and bar came loose and employee jumped form about 5' and hurt foot which resulted in fracture of right foot.
While walking on wet concrete, employee slipped and fell to the ground.
Employee was climbing down off a haul truck when he stepped onto a large mound of gob and twisted his right ankle. Lost time injury.
Employee was walking along a belt when a piece of coal bounced off the belt and landed on top of his left foot causing a fracture to his foot.
EE was helping install rear end in truck. Felt pain in shoulder while lifting rear end. Continued to work full time until scheduled for surgery. Will be off work for 3-4 months for recovery.
While returning from south-side loading to inspect and repair the chain for the scope positioner, EE caught his foot on a step and tripped and fell about four steps to the ground straining his left ankle. Employee was taken to the ER where he was treated and released. Employee is on prescription pain killers and crutches.
Employee was on the second step of a step platform 16 inches off the ground, he turned to lift down a section of 3 inch pipe that had been cut away from the main line. As he turned his right foot stepped on his left foot causing him to slip off the step, his left foot caught under the step twisting his left leg. He felt pain in his left shin, transported to hospital.
Someone had broken leg off scale. Employee used a shovel to pry up the scale to put leg back on. The shovel shifted & came out. Employee's hand was under scale when it dropped.
Employee was clearing a blockage in the drill bit of a hydraulic track drill. While unthreading the drill head, the drill stem came loose, causing the entire section to fall and crush his right ring finger.
EE was attempting to hook up to a 4' X 6' utility trailer to remove some old wiring. The EE needed about 1 inch to set the trailer to the receiver hitch; EE lifted the trailer by hand to the receiver hitch when incident occurred.
EE was sandblasting a pump body. As he flipped the pump body over, he felt a ""twing"" in his lower back. Minimal pain at the time. On 01.04.2010, EE was placed on restricted duty and issued pain medication.
In the process of grouting new base for a replacement pump. He was mixing grout in small 5 gal portable mixer. Finished mixing up a batch, shut off mixer tried to remove pail from mixer but pail was stuck. While trying to free pail mixer started up: his hand spun round and round causing a sprain and possible fracture. Fracture ruled out at a later date. The sprain required a splint
EE was walking up the slope of an empty settlement pond. Employee felt a cramp in the back of his right leg. Employee was taken to the clinic for diagnosis. Injury was diagnosed as a strain in the hamstring area of his right leg. Employee was placed on light duty for 7 days by the attending doctor.
EE was in the process of installing bracing to support rebar column on the pre-heater building. As EE was supporting the brace he stated he felt a pain in his left shoulder.
Employee was standing upright and tightening a bolt on the crusher. Employee states that he felt a twinge in his back while performing this duty.
EE was hosing down with caustic solution when he felt a burning sensation on the top of his right foot. EE removed his boot and discovered that caustic had soaked into his sock from a leak in the boot. EE was then sent to medical where he was treated for 2nd Degree Chemical Burns to the top of his right foot; resulting in a recordable incident with restrictions.
Employee was using a 4 1/2"" grinder with a cutoff disc attached to dress a bolt on a flange. While cutting excess metal from the flange, the cutoff disc jammed or caught on the flange, causing a portion of the cutoff disc to disintegrate and strike the employee causing a laceration to his left leg.
Employee was digging out the # 1 three inch feeder with a digging bar, a loader was digging out # 2 feeder when material on the northwest edge of feeder #1 sloughed off and caught his legs causing a right knee strain.
While walking from work area at end of shift, ee states he was knocked to the ground by co-worker who fell. EE received medical treatment for pain to left hip, back and left elbow contusion and back strain and was put on modified duty 12/29/09. No time has been lost.
Injury strained his back using a shovel to pry a block of ice out of a haul truck.
Employee was mucking out the settling ponds at the mill using a longreach trackhoe. He caught a ride back to the quarry garage on a waste truck. When he exited the waste truck, he slipped on the steps, lost his hand hold and fell to the ground on his rear. The fall was from the top step approximately three feet from ground level.
While getting into the back of a mantrip, employee fell landing on his back striking his head against the ground. Employee needed several sutures to close the wound on his head and was placed on restricted duty.
While working on a track dozer, the technician stood up from a squatting position on the concrete shop floor and turned to his left. The twisting motion fractured the fibula just above the right ankle.
Cutting screen side & knocked bolt out, screen moved/shifted & pull chain wrapped around hand.
Employee was standing beside the electric water pump while it was running when the 2"" ball valve came apart on top of the pump. He was sprayed with water and then the cam lock end of the discharge hose came apart and hit him in the nose. The cam lock struck him in the safety glasses and nose. He received a cut on each side of his nose and his nose was broken.
Employee was searching for a hose in the storage container located in the load-out department when he sustained insect bites on his hands and feet. As a result, the insect bites aggravated a few days later and his foot became swollen.
EE was welding in a kneeling position in an additive tunnel. A co-worker walking past EE lost his footing and grabbed an unsecure angle iron conveyor brace to stop his fall. The co-worker fell across the EE's upper back and shoulders with the angle iron brace.
While assisiting the rail load-out operator to close frozen doors on the bottom of DVP open hopper cars, EE's middle finger on right hand was pinched on the epad portion at the end. He was wearing all the proper PPE for the job and weather conditions.
Employee slipped on wet/icy roadway and put their right hand down to catch themselves, jamming their wrist. Employee was diagnosed with right elbow lateral epicondylitis (right wrist sprain). His work restrictions made this event reportable.
Employee was emptying load from shuttle car to crusher feeder, when a rock flew up against canopy post and pinched employee's finger. Employee received medical treatment for injury.
Employee was moving a 55 gallon drum of oil, when the drum slipped off of the pallet. Employee tilted the drum to remove his foot and felt pain in his lower back and right shoulder.
EE was helping two other employees tension cable bolts using a hydraulic tensioner. The cable was bent and had bound inside the tensioner. A 4 lb hammer was being used to the knock down on the top of the tensioner as EE pulled downward on the tensioner handles. On the 3rd or 4th hit, the hammer deflected off and struck the employees finger.
EE was cutting floor grating with grinding wheel. The wheel hit the edge of grating and kicked, resulting in a cut on the side of the left hand.
Employee was pulling her tool box to put it away when she smashed her hand between the tool box and a haul truck tire.
While pulling basket filters from ball mill sump filter employee strained left wrist and back.
Operator drove front left tire of a ten wheeled gravel truck into a hole covered with ice causing the truck to roll on its left side.
Bucket Wheel Mechanic (BWE1),with thirty five years experience, was removing bolts from a brake pack/adapter plate secured in a vise on the back of a work truck. When the vise pressure was released to rotate the position of the assembly the unit rolled out of the vise pinning left index finger between the adapter plate & the truck bed. BWE1 was transported to First Aid.
Equipment operator stepped into a depression in the ground which aggravated his back. Emergency room doctors determined that the employee had a preexisting degenerative lumbar disk condition which was aggravated when stepping from the excavator into the depression. No workdays were missed, but the Dr. placed the employee on limited duty for recovery.
Lifted stone to throw away and twisted back.
Injured was standing in an upright position and was reaching above his head to hang a ventilation line curtain, when he fell back against the continuous miner which caused a pain in his back. Injured continued to perform his normal duties, without restrictions, and began to perform restricted work on 12/28/2009 making it reportable on that date.
Employee was using a scaling gun at 30-7 sump. The bit got stuck in scale. As he attempted to pull the bit out of the scale, he felt a pop in his lower back. Lower back strain; placed on restricted duty.
Excavator was shutting down at end of shift. Excavator was top loading trucks, the operator cut a ramp to walk excavator off the level it had been operating from. While walking down ramp, excavator started bogging in dirt and suddendly stopped, caused cab to tilt over. EE released seat belt, hit forehead on windshield, deep cut. 2 witnesses called foreman(called 911).
Employee was doing some repair work on the leach pad and while walking he slipped and fell on some ice. He strained his wrist, shoulder, and a possible scapula fracture.
Was in a man basket and reached out to weld a bracket when he felt a pull on his left knee. Knee started to bother him a few months after the incident.
EE WAS HOSING DOWN WITH LIQUOR. EE HAD THE HOSE BETWEEN HIS LEGS, WHEN THE HOSE SLIPPED, AND EE GRABBED THE HOSE NOZZLE. THE NOZZLE SPRAY KNOCKED OFF THE EE'S HARD HAT, FACE SHIELD, GOGGLES AND GLASSES. EE WAS SPRAYED WITH LIQUOR IN BOTH EYES. EE FLUSHED HIS EYES AT THE SAFETY SHOWER IN THE AREA. EE WAS TREATED BY PLANT MEDICAL STAFF. EE PLACED ON RESTRICTED DUTY.
Employee stated he was walking over to the left side of the section, when he stepped in a hole twisting his right ankle. He also fell on his face and blooded his nose. Did not miss any work but was put on transitional duty for 1 day.
Employee slammed Left knee into Grease Zerk on the Mucker causing small puncture wound to his knee.
Employee was walking across the deck of HT 491. He stepped up on the platform with his right leg, and caught his left foot on the step. He started to fall, but caught himself, twisting his right knee.
EE lost his footing while stepping down off an isolation base from painting a piece of pipe, causing him to catch himself with his left wrist and bumping his left elbow.
Pulling on pipe wrench and strained right shoulder. First aid treatment applied initially. Patient was not getting better so work restrictions applied on 1/8/2010 which changed the case to restricted work.
Area he was in was full of vapor from the start up of bank 2 dissolvers; stepped into the pump area of building & stepped into a ditch full of hot water.
An employee was attemping to tension a chain and place it onto a hook. During this process the employee got his finger pinched between the hook and chain. The tip of the finger was cut and the finger nail was torn off. The employee received a tetnus shot, antibiotic shot and seven stitches.
EE was operating bolt machine while putting the last roll in the #4 heading when a piece of rock fell from the t-bar mashing his right hand on the drill pot. The rock measured 3' long x 10"" wide and 8"" thick.
Employee was struck by another employee with a hammer while working to splice conveyor belt. Smashed middle finger of left hand. Dr. treatement for a fractured finger.
Employee was hit in the right ear by an air gun that flew out of a co-workers grasp causing laceration behind right ear requiring 5 stitches.
Employee was about to cut a wear plate that was plugging a feeder. As he turned on the oxygen and acetylene to the torch system and lit the system, a fire ball came from the connection and struck his left palm.
Leadman was closing up the #5 preheater prior to startup for the next shift. EE began to close the 2'x2' cast door on the west of the ram floor and had to reopen it to remove access planks inside. While opening door to his right his right hand middle finger got caught against a fixed ladder causing an open fracture requiring medical attention and temp. restricted duty to heal.
WHILE GUIDING OUT WINCH TRACTOR CABLE; CABLE TWISTED HITTING INJURED EMPLOYEE
While ee was using air lance to clean out feed end/seal housing, finger was pinched between air lance and door frame. Left pinkie finger had to have 3 stitches.
EE was shoveling hard/frozen slurry and hit shovel hard and heard snap in hand. EE having trouble closing his hand.
Employee was pulling freight cart w/200-300lbs. of UPS deliveries. Employee lifted her R/leg and turned towards right side pulling the cart and when she put her weight on her left leg, employee felt pain in left knee cap area.
EE hit a d-7 rail link with a 5lb hammer this caused a piece of shrapnel to fly and lodge under injured EE's skin at the right temple. The piece of metal was 1/4-1/2 inch in length
Employee was bending over working on a bearing of a stone elevator for several minutes. When he straightened up, he felt a sharp pain in his lower back.
While walking to the sample bin he stepped in a hole and hurt his left foot. An x-ray showed a small fracture on a bone in his left foot and he was placed on light duty. Employee was also given a splint.
Employee was using a volt meter to test the alternator voltage on a 16 G motor grader. His foot slipped causing him to lose balance. As he tried to catch himself his left hand contacted the fan, resulting in multiple fractures and lacerations of his hand and fingers.
Employee was installing a roof bolt. He spun the chuck to line up bolt. His glove caught between bolt & chuck. Glove twisted, dislocating his left thumb.
Employee was Roof Bolting and was lowering drill head when he inadvertently lowered head of Bolter on right foot. Employee did not miss any work however he did seek medical attention and was placed on restricted duty making this reportable.
Employee alleges he strained his right groin area as he slipped while sweeping clinker and did a half spllit. He failed to report this accident until 12-15-09 at 8:50 AM. He was sent to the Industrial Clinic and was placed on restricted work duty. This alleged incident is being investigated as to whether or not it is work related or an incident that may have happened at home.
Employee was climbing down a ladder, missed a step and fell, landing on right wrist/forearm. Wrist is broken and splinted/casted, limited use of right hand and arm.
Employee stated he was attempting to install a 6'roof bolt. He had inserted the bolt into the drilled hole and started pushing on it to straighten it. As he was pushing away he felt pain in his lower back. Employee was treated in the ER and returned to work on transitional duty.
Emp was loosening a fitting on a water line a miner. He was using two end wrenches and pushing them inward. He felt pain in his right shoulder. Took to company doctor and was placed on medication, restrictions, and therapy.
The company provided transportation was stuck due to icy conditions. The employee got off the bus to see if she could help. When the employee realized that there were enough people to help, the employee started to get back on the bus and while doing so the employee slipped on some ice and fell to the ground.
The 345D excavator was low on hydraulic oil and needed to be topped off. While attempting to remove the Hyd. oil cap (which was on very tight) with a ratchet wrench the ratchet wrench malunctioned. The momentum of the slipped wrench caused the associate to fall backwards to the ground on a rocky surface. This resulted in the asociate breaking the bone in his left arm
EE slipped on a ladder and left knee hit a beam while holding a beam for a welder. INCIDENT BECAME REPORTABLE WHEN HIS KNEE WAS IMMOBILIZED, ON 1/6/2010, WITH A BRACE.
Hand got caught in pinch point while moving a valve. Pinched 5th digit right hand. Work restrictions 1st applied on 12/15/2009 causing the case to change from first aid to restricted work.
Employee was removing a steel door from a pump box pinning the right little finger between the door and hopper.
Injured EE and another EE were setting timbers underground, injured EE strained a muscle in his shoulder while lifting timbers.
Employee was dragging a screen deck across the ground and strained his right shoulder.
EE states he was caring a box that had blown away due to high wind and while walking with the box a high gust of wind caused the ee to lose his balance and fall. EE fractured his left wrist when he pt his hand out to brace himself for fall. EE on modified duty.
While moving a rubber fas vac hose the employee felt a slight pull in his lower back.
