Accident type, without injuries

A roof fall occurred in the 1st East Submain in the #7 entry at spad #40+40 measuring approximately 30 ft. long x 18ft. wide X 9ft. high. The area was supported with 4ft. fully grouted bolts and 8ft. cable bolts.

The 8' hoist was found to be having an electrical problem that prevented it from moving up or down without tripping out. 2 employees were on the skip at the time performing normal maintenance. After discussions with the maker and with MSHA Inspector George Olivier, the skip was lowered to the 1300' level using the braking system, the employees then came to the surface

A roof fall occurred in the intake air course of the #2 entry of the mains section at survey station #1156. The roof fall did not affect the ventilation or the passage of miners. The fall measured 30 x 65 x 5' 6"" & the area was bolted with fully grouted rods. The fall is a result of the stress applied to weak planes in the thin bedded laminated shale strata over a period of time.

While working in the 8' shaft the hoist tripped and would not lift the skip up to surface. They were at the 400' location. After several attempts were made to bring the men up failed, the men were egressed down to the 1300' level and got off the skip. No one was injured.

The Kulntown elevator went down with 15 persons on board. The people were manually brought to the surface and released at 4:27pm. The problem was electrical with the encoder. Elevator back in service 6:15pm.

Traveled Return Entry A fall in #2 entry. Airway was NOT blocked. Travel ways was routed to the #1 entry. The area of the fall was dangered off. MSHA/State was notified via hotlines. Area was then cribbed, roof jacks set and Danger Boards were put in place. The fall was 4-5 feet thick 20 - 25 feet long- 18 feet wide. safety talk with all three shifts.

A fall of the roof occurred on the 1st 54 S.E. Mains crosscut 56 between the supply road and brattice. 30' length by 18' width by 6'-7' high.

A roof fall occurred in the #11 seam in the 2 Main North between crosscut 98 and 99 in the #4 entry. The fall measured approximately 50'x8'x5'. The area was supported with 30"" fully grouted roof bolts. An undetected slip was determined to be the cause of the fall.

Windy conditions possibly caused a brush pile to reignite and embers were blown over a fire lane. The under brush of 1 acre of wooded area was burned. No miners were in the area.

At approximately 11:15pm the elevator stopped at mid shaft. After checking the control room of the elevator the elevator showed fault. The fault was due to the magnet on the north gate of the elevator. The elevator was put back in service at approximately 1:40am.

An unplanned roof fall was discovered in the intake of the 1st room set-up off of the 3rd South Mains. The location of the fall is at spad #9+80 between entries 8 through 10 in the cross-cut. The fall measured app. 60' long x 19' wide x 6'-8' high. This area is app. 800' from the #1 unit(MMU 001-0).

Kuhmtown elevator went down at 8:04am with 20 personal on board. People were lowered to the bottom and safely released. Rope dogs actuated and encoder. Elevator back in service at 10:10am. Encoder changed and speed adjusted.

An unintentional fall of the mine roof was discovered in the Cedar Grove Seam, #2 Entry, #27 X-Cut. The fall meas. approx. 6' H., 18' W., 19' L. The fall was immediately reported to MSHA and the State by phone.

The Bailey (BMX Portal) Elevator was out of service due to an electrical problem involving a oil cooling fan. No person were injured due to this event. The elevator was fixed and back in service at 8:30am.

An unintentional roof fall occurred in the 5B intake @ crosscut 114 in the #3 entry. The fall measured 24'x 22'x 9' thick. The fall did not affect ventilation, passage of men, or cause personal injury. The area had been supported with both 4' and 6' fully grouted roof bolts. The fall will not be cleaned up.

An unintentional roof fall was discovered at approximately 1:00pm in the left return on Grassy Creek Mains, at crosscut No. 13 adjacent to survey station No. 138. The fall was approximately 8'high, 20'wide, and approximately 20' long. All approaches to the fall were timbered off. The fall did not interrupt ventilation and will not be cleaned up.

A fall of material from the roof of the K2 tunnel resulted in damage to the K2 conveyor. No miners were injured in the incident. The incident was called into MSHA via the 800 number as an immediately reportable accident.

A section roof fell outby the MRS units causing them to be trapped. The fall was located in the 4 LT panel inby the 19th x-cut in the #2 entry. The area was previously supported with 5' HY-Tech bolts and 12' cable bolts. There was no persons or equipment involved with the fall.

Roof fall at crosscut #1 of the 7th Right Mains Section. The fall was in the #3 beltline entry. Dimensions are approximately 19 feet, by 19 feet and approximately 13 feet high.

A roof fall occurred in the #4 entry spad # +520 of the 1st NE parallel 1st rooms. The fall was just in the intersection and appro. 6 plus feet high. The fall is appro. 19,000ft from the nearest section.

While retreat mining was being conducted in the #4 entry inby the 19th x-cut lithe 4 LT panel, a section of roof fell outby the MRS units. The units were could not be moved outby because of the fall. The fall measured 10' long by 18' wide and was 5' in thickness. There were no injuries.

An unintentional roof fall was discovered on the #2 section, Entry #7, Near Spad #463. The fall measured approximately 35ft. long x 55ft. wide x 5ft. high. No injuries resulted from this fall.

A roof fall occurred in the #9 seam in the 1st Southwest Mains at crosscut #20 in the #4 entry. The fall measured approximately 70'x18'x6'-8'. The area was supported with 6' tension rebar roof bolts and the intersections were supported with 9' double lock mega bolts.

A roof fall occurred at 7:05 A.M. on the Head gate Unit 001. In the number 3 entry in the intersection at x-cut number 144. The roof fall was 19 foot wide by 19 foot long, about 10 foot high.

An unplanned roof fall occurred, starting 2 breaks out by Survey Station 1632 of #2 Section between #1 and #7 entries and continuing to the section face. No injuries or entrapment involved, no ventilation involved. Secondary escapeway blocked. Fall was approximately 7 1/2 ft height x 20 1/2' wide x 40 ft. length.

#11 Seam 1st northeast panel spad #28+00 cross cut 35,#7 entry, an outby fall occurred in the intersection. The intersection was pinned with 6' bolts. The fall was about 6' high. The fall was reported to the MSHA HOTLINE.

A roof fall was discovered in the #9 seam 4S panel, #2entry, xc-26 and spad #17 + 56. The approximate size of the fall was length-30ft, width-20ft and height-8ft plus.

A roof fall has occurred at 8 Mains in the old works at break 20, between 3 and 4 entry no ventilation or equipment effected, the fall is approx. 60' long and 8' to 10' thick. The area is dangered off and operator has proposed to seal off entire area.

AN UNPLANNED ROOF FALL OCCURRED ON THE #8 SOUTH BELT AT X-CUT #30. THE AREA WAS MINED AROUND OCTOBER 2007. THE FALL WAS ABOUT 6 TO 7 FOOT HIGH, 20 FOOT WIDE, AND 100 FOOT LONG. THE FALL WENT FROM #4 ENTRY TO #5 ENTRY ON THE BELT LINE IN THE INTERSECTION. THE INTERSECTION WILL BE CLEAN UP AND REBOLTED. ALL OTHER APPROACHES WILL BE CRIBBED OFF.

The day shift crew was close to the operating section when it heard a fall occur on the belt line one break outby the section tailpiece near spad 15538. The fall was 5' high, 20' wide and 35' long. The area was dangered off, timbered and the section was moved. 48"" resign bolts were used. No injuries were reported. State and Federal agencies were notified.

A rod near the brake shoe of the elevator was bent. This tripped the safety on the elevator causing it to stop.

A rock fall was discovered at 9:21pm on the #1 Left Mains in the #4 entry at cross cut #7. The fall measured approximately 20' wide x 20' long x 7'-10' thick.

A roof fall was discovered on this date. The fall was located on the Slope beltline from break 43 to 44. The fall measured 20' wide by 100' long and was 2-5 feet in thickness. The fall was cleaned up and re-supported. The area was originally supported with 4' fully grouted rods. There were no persons involved.

At approximately 100 Ft. down the elevator stopped with two miners aboard. The miners were stuck for approximately 1 hour at this location.

A roof fall occured in the 3rd east main entries at x-cut #31 in #6 entry. The fall measured 35'x 19'x 8'. The fall was caused by a hidden slip in the mine roof. The area was mined in May 2007

THE MAIN PRODUCTION HOIST WAS TAKEN OUT OF SERVICE THE MORNING OF 10/21/2011 AT 7:00AM TO PERFORM HYDRAULIC SYSTEM MAINTENANCE. AT 6:05AM, THE MSHA 1-800# WAS CALLED BY AN EE TO ADVISE THEM OF THE SITUATION. THE HOIST WAS PLACED BACK INTO SERVICE AT 5:40PM. NO INJURIES OR ACCIDENTS ONLY THE HYDRAULIC MAINTENANCE WAS PERFORMED.

The emergency shaft was cleared with an empty inspection cage. Next empty trip down, the cage did not reach bottom. The cage was hoisted back to the suface. The hoist cable was under the cage and tied into a knot. The hoist rope was damaged and will need to be replaced. The shaft will be inspected for obstructions and necessary repairs made before entering the mine to work.

Roof fall was found at 9:55am inby spad #750 in the #1 entry MSHA was called at 10:02am it has been cribbed off and dangered off.

The elevator in # 6 airshaft was inoperable. The emergency stop button needed to be replaced. The elevator remained inoperable until 11:20 p.m.

A roof fall occurred in the #9 seam in the 1st Southwest Mains at crosscut #31 in between #2 and #3 entries. The fall measured approximately 40'x18'x10'. The area was supported with 6' tension rebar roof bolts.

Before the weekly inspection of Lyons Salt's emergency shaft, it was determined there was not communication between the surface hoist and shaft bottom. No one was injured. The phone at the emergency hoist did not work.

An unintentional roof fall occurred on the 5B belt line at crosscut 99 in the #4 entry. The fall measured 22'x 25'x 8' and will be cleaned up. The fall did not affect ventilation, passage of men or cause personal injury. The area had been bolted with both 4 and 6 foot fully grouted roof bolts and had timbers in the intersection.

A non-injury unintentional roof fall occurred on 2 North intake for MMU 002-0 section. The fall was in #8 entry at cross-cut 48 on 2 North Mains in an intersection. The fall was approximately 10 feet thick, 70 feet wide and 20 feet long. This area was initially supported with 60"" fully grouted rods.

The Kuhntown elevator went down with 6 persons onboard. Elevator was lowered to the bottom location and persons were released (5 min). Problem was bad end coadu. Elevator back in service 5:00 am.

MAIN PRODUCTION HOIST WOULD NOT START AT BEGINNING OF SHIFT. TROUBLE SHOOTING AND INSPECTION WAS PERFORMED. DISCOVERED A BAD BEARING THAT REQUIRED REPLACEMENT. BEARING WAS REPLACED 10/20/2011.

Service hoist cage was chaired at shaft collar, when hoist man attempted to move cage upward, DC breaker tripped and would not reset. Initial evaluation indicated hoist would be down more than 30 min. UG employees evacuated safely. SCR failure, components were 30+ years old. Common SCR's from an out of service unit were used to restore function.

An unintentional roof fall occurred at the New Future Mine 1W Longwall Entry #3 between Crosscut 12 and Crosscut 13 (1574W to 1599W). The fall was approximately 25 feet in length, approximately 17 feet in width and approximately 16 feet thick. The fall will be cleaned up. Fall #28G

A unplanned roof fall occurred on the #2 section in the #3 entry at spad 6839. Fall occurred 80' from the active #2 section. Fall measured 20'x20'x7' area was initially supported with 5' torque tension and 8' cable bolts. Ventilation was not affected, passage of person was affected. Area will be rehabilitated. Federal and State agencies were notified immediately.

A 1000 volt fuse blew on the Production Hoist. The Production shaft is the 2nd way out of the mine. Hoist was not operational until 4:35 pm. so the miners had to be evacuated out of the mine until the hoist was running again.

A roof fall occurred in the #5 entry spad # +3606 of the Cypress Creek main. The fall was just in the intersection and appro. 6 plus feet high. the fall is appro. 2500ft from the nearest section.

The elevator in # 6 airshaft was inoperable due to malfunction in the TAC pickup circuit. 15 people were trapped for approximately 20 minutes until the electrician responded. The elevator remained inoperable until 7:45 pm

A weekly examiner found a roof fall adjacent to the weekly examiners rout of travel. The fall is located in our M4 Mains, x-cut 63, entry #2. The fall did not affect ventilation, impede miners travel, or cause injury to miners. The fall measured 17' x 17' x 8'.

At about 9:30 AM an employee started a fire in the Earth Stove in the shop. The stove pipe was defective and subsequently started an un-contained fire at the back of the shop. The incident was reported to MSHA in a timely manner. The maintenance shop was a total loss. There were no rules violated, no mining equipment involved, no related job skills and did not involve PPE.

A Non-injury mountain bump occurred in the 4 north, mmu, 010, longwall tailgate entries. From the longwall face outby for 60 feet in the #3 entry between 28 and 29 x-cut and inby 28 x-cut in the #2 entry of 3 north.

A seismic event occurred near the 185 raise on 4900 that dislodged approximately 1/2 to 2/3 of a car of muck from the raise that was deposited at the bottom.

#4 Main's #1 Hdg. pumped water out of old sump area, and found a roof fall. Fall was 30' long 8' high 12' wide, and was supported with 7' tension rebar bolts, had 680' of cover, was mined Feb 09, was 50' inby spad 7281 in #1 Hdg.

A roof fall was discovered by an examiner in an outby intersection if the #5 entry in the 4 left section. The size of the fall were approx. 25' l. X 18' w. X6-8' h. The roof was supported with 6' fully grouted resin bolts. There were no visible defects in the area. All accesses to the fall were cribbed off.

The electrical power source for the Yards & Docks elevator failed while an employee was riding in the elevator car. He was stuck in the elevator while the electricians worked to restore power. There were no injuries involved.

A roof fall was found 10/14/11 at xc 86 entry 4 in 6R. This was adjacent to travel way in a four way intersection that had been mined 8/2009. Area was bolted with 8' full resin pins. No injury or property damage resulted. There are no plans to clean up this fall. Fall was approx. 20'W.X 30'L. and 10' H.

A roof fall occured in a 3-way intersection in #1 entry at spad no. 11248, measuring 150' x 20' x 12' thick. The area was roof bolted with 3/4"" x 6' tension rebar roof bolts with several 10' long cable bolts with T3 channel.

A roof fall occurred in the 1st main west supply road in the #7 entry. No people or equipment was involve there were also no disruptions to ventilation. The fall was approx. 15' H. 20'W. 20' L. It occurred at approx 5:00p.m.

A roof fall occurred @ 30+40 in a three way intersection on the south mains intake, #7 entry there was already some cribs set, the fall fell from the rib line to the cribs. the fall was 6' wide by 27' long 2 to 6' high.

We found a roof fall @ spad #29+00 in a cross cut 4-5 in the South Mains, the fall was approximately 26' long x 18' wide x 6'high.

A roof fall was discovered in the old abandoned 002 area. The fall is located in the #4 entry. The fall was apx. 40 feet long X 20 feet Wide X 10-12 feet high. No persons, ventilation nor equipment were affected by the fall.

A non-injury roof fall was discovered on the MMU No. 003-0 Section. The roof fall was immediately reported to MSHA hotline. The roof fall is located at CC #7 of the 2-North West mains. The roof fall began in the #5 entry and cont'd. through the #7 CC into the #6 entry. The roof fall is approx. 90-95' l. x 5-15' w. x approx. 6-8' h.

The underground trainer was in the process of training a new employee on the secondary Alimak escape way. They started back down when the Alimak stopped on the rail. The underground trainer went to the lower deck to start trouble shooting the problem. Upon further investigation it was determined the Centrifugal brake system had a brake shoe failure.

A roof fall occured in a 4-way intersection measuring 70' x 20' x 12' thick. The area was roof bolted with 3/4"" x 6' tension rebar roof bolts with a few 8' to 12' long combination roof bolts supplemented in.

During the Operator Safety checks on the Hoist system the operator went to move the skips and two fuses blown. There were no more fuses on the property. Failed SCR was detected. The number 2 quad module had one SCR with an open circuit on the gated side. This module is on the same incoming power as modules 7 and 8 With one failed SCR not firing unable to complete proper waveform.

A roof fall occurred in a 4-way intersection at spad no. 11278 measuring 60' x 20' x 12' in thickness. The area was bolted with 3/4"" x 6' tension roof bolts. At least 8 - 10' long cable bolts were installed in the area also. No injuries occurred.

Roof fall occurred 10/11/11 on the main East #11 Belt at the 11-30 track to belt cross cut. The fall is approx. 80' L.X 21"" W. X 15' H.

A roof fall was discovered at 3:50 pm in the #2 entry of the SE Mains 40' inby spad # 36. The fall measured 60' long by 20' wide by 6' thick. The fall does not impede ventilation or travel and has not been cleaned up. The fall has been breakered and dangered off. This area was mined in 2006 and was supported with 4' fully grouted bolts.

An unintentional roof fall was discovered in the Old Mains Section, Entry #1, approximately 20' inby spad # 6544. The fall measured approximately 70'L x 20'W x 6'H. No injuries resulted from this fall.

Mine foreman was making routine mine checks in affected area around 12:50 a.m. When he entered Mod 12, room 8 he found that an area of the roof had fallen. The area was approx. 60' X 75' X 12' thick. No machinery or personnel were in the area at the time of the fall.

At 7:50 am the hoist at the Hart Street Portal failed to operate. At this time it is unknown the cause. The Secondary escape way has been re-routed to the South Portal.

Blue Creek #1 Mine discovered an unplanned roof fall at 12:40pm. The fall was located at 11 break, #7 entry intersection on Mains #3 belt outby the working section. No injuries occurred and ventilation nor travel were blocked by this fall. The fall measured approximately 20' long x 20' wide x 10' high.

A roof fall was found by pre-shift examiner at 7:00am on Oct 8, 2011. The fall was in a four way intersection at xc 29 main south, blocking access to #10 main east seal. The fall is approx 20'wide X 100'long X 10'high. Area was mined Sept of 1983, and had been supported with 8' full resin pins. No plans to clean fall, once MSHA approves a plan work will begin on something

Burlington Northern Railroad was doing repair work on their tracks and property when they caught the grass on fire that spread onto mine property. Emergency services were called and the fire was put out but small spot fires continued to pop up over the next hour due to the wind and dry conditions. The fires were far from plant operations.

A roof fall occurred in the 1st Main East, #8 entry at XC 32 no injuries or disruptions to ventilation. The fall was approx. 18' long - 18' wide - 8' high. The fall was discovered around 1:00 AM. There was also no equipment involved.

A roof fall was discovered in the 7B off 5 north miner section. The fall was in the track entry in the four way intersection at 25 wall. The fall was 25 feet long by 16 feet wide by 13 feet high.

The 5 north escapeway hoist was removed from service due to a bad limit switch on the hoist. Portable hoist truck placed on site. Hoist was repaired by tensioning chain on 10/7/11 at 10:40 A.M.

A gas leak was discovered at a gas well near the ramp road to the pit. The well company was called to investigate. MSHA was also called at 8:33pm. When the gas company arrived they found no leak. The gas company stated the gas release was probably caused by a relief valve that can sometimes open for a short time then re-seal.

A roof fall occurred on 5 section crosscut inby spad #11005 in the #5 entry. The fall measured 6' H. X 20' L. X 20' W. No one was injured. The fall was cribbed off, and dangered off. MSHA and State has been notified. The time it was found was 3:00 p.m.

Due to a wild fire starting on BLM property and burning onto Goldstrike property, the Meikle/Rodeo underground was evacuated as a precaution due to power bumps from electrical lines in the path of the fire. EE was taken to a local optometrist to remove the piece of dust from his eye. EE was told to stay home for 2 days to allow eye to heal.

Employee dropped the blade, as it hit the ground, rod trailer caused skidder to start to jack knife he then chose to unbuckle and jump from skidder. Employee was examined at the local hospital where a hairline fracture of his ankle was revealed. Employee was released to return to work. Struck his hard hat against a low hanging roof strap while walking resulting in neck/shoulder pain. Starting missing work on 11/4/11 and was diagnosed on 11/29/11 for spinal damage.

Employee came out from underground and went to doctor, where he received staples and then was released back to work. Employee still complained of pain, went to Comp Center the next day where he was seen by a different doctor and ordered to stay off work until next visit on October 14th.

While stepping out of a motor, with one foot on the ground, the employee caught left foot on the safety chain causing him to lose his balance. He fell to the ground and twisted his right knee. An MRI indicated a meniscus tear. The employee's first day off of work was 2/7/11.

While cleaning the dozer the injured employee scraped left forearm. He continued to work his shift but the following day he was seen by his physician and excused from work for two days. He was excused from work due to the risk of infection secondary to history of cellulites. He returned to work on 10/10/11.

He bent down to lift up the lids and while lifting turned slightly. Upon turning he felt a pull in the lower left side of his back and dropped down to one knee in pain. The EE was transported to area hospital for medical evaluation.

Employee was working in the primary feeder hopper. He was standing on built up material in the hopper, clamping a piece of belting up so access to the rotor was open. He turned to climb down and his foot slipped, causing him to fall sideways about 2 ft into the bottom of the feeder trough, impacting his right shoulder.

Employee was the scoop operator, he was trying to pull a shuttle car off the feeder. He ran his scoop bucket on top of the shuttle car and the scoop dropped off the shuttle car and bounced employee inside the cab of the scoop, he hit his upper left rib cavity on the inside of the scoop cab causing a pulled ligament.

He opened the spring latch door to the man-lift and used his hip to hold the spring gate open as he backed into the basket. Below a welder was preparing to board the shovel. The welder started lowering the basket just as the electrician stepped back. The electrician fell to the ground.

The man lift dropped suddenly striking the mechanic in the left shoulder. Leaving a deep tissue bruise in the upper left arm.

The victim operating the helper side on a twin boom bolter was drilling a hole for a corner bolt. When he reached for his drill steels a rock measuring 8"" x 1' x 2"" thick fell from the roof and struck his finger.

It was reported that the brakes on an end dump malfunctioned as it traveled down the haul causing the end dump to hit the front side of the loader. The loader sustained moderate damage. A conveyor transfer had plugged and spilled material around the tail section. The area was being cleaned up by the production crew utilizing manual labor and a mini skid-steer. A 3-ft section slip-lock PVC piping was attempted to be pulled from an elbow to provide add'l working room. Unfortunately, the pipe slip-joint connection was stronger than perceived by the miner.

Over-exertion (Not Elsewhere Classified)

At the end of the shift 10/13/11 EE reported to me that he had twisted his knee. I asked if he was injured, he told me no, he was just letting me know what happen in case it got worse. He was Climbing down the ladder of his haul truck and his knee bent the wrong way. He told me that he didn't need Medical attention, so we agreed to see what came of it.

Over-exertion (Not Elsewhere Classified)

Employee said he was lifting a door and heard a pop sound. He did not use proper lifting type of tool for task attempted. The medical quantification is unknown and will be determined by professional medical staff.

Struck by flying object

Employee was installing a cable bolt when it became jammed and struck employee in the face. Lost time injury.

Struck by falling object

Operator was getting ready to start pinning when a piece of the rib rolled off, knocking him down and pinning right leg. ***Began missing on 11-14-11.***

Struck against stationary object

The employee stated he injured his shoulder when attempting to dodge a falling rib roll. He stated he jumped and contacted the RBM with his left shoulder.

Over-exertion in lifting objects

Employee was lifting a pipe that was 6' long, 2"" diameter, and approximately 40 lbs.

Fall from ladders

Employee reported that while putting away stock, she missed the last step of a step ladder she was using. She pulled the ladder down with her and fell on her back and jammed her right thumb.

Upon completion of a pipe replacement the EE slid from the pipe rack to the cat walk. Once standing he twisted to move forward and felt a pop in the knee.

Employee was aligning the load spout to the Railcar. He was aligning the spout with his right leg. He was supporting his posture and gripping the hand rail. As he pushed with his right leg. He felt a sharp pain run through the back side of his right upper leg which resulted in a torn hamstring muscle.

Employee bent over to hook fuel hose on the service truck, when he stood up he felt pain in his lower back.

Struck by falling object

A crew was trying to assemble a belt drive system. Discharge boom needed adjusted to align into drive frame. While adjusting, the discharge boom dropped striking an employee's left foot (fracture).

The man was carrying oil cans when he stepped in a hole in the bottom and twisted his knee. The man continued to perform his regular duties until 10-20-11 at which time he went on restricted duty status.

Fall to the walkway or working surface

EE was cutting a piece of plate that was near the remaining bulk of the plate he was stepping backwards and his foot stepped into a gap on the plate, causing him to fall to his side with his foot lodged in the hole he had stepped in.
