Accident type, without injuries

A roof fall measuring 20' long, 20' wide and 9' thick was discovered inby spad 2938 # 2 entry in the old # #6 Mains. The fall was 3600' outby the working section, ventilation, equipment, and the travel of personnel was not affected. No injuries occurred from the fall, the type of roof support in the area was 48"" resin bolts.

A non-injury roof fall occurred in the old # 1 section (001-0 MMU) of this mine, at spad # 246. The fall was approximately 5' to 6'H X 18'W X 40'L. The fall has been breakered off.

A loader operator was feeding a portable screen plant, the loader operator dumped his load into the screen, and started backing up, not realizing another employee in a company pickup had pulled into the area. The loader operator did not see the company truck and backed into it. This resulted in the truck door receiving damage. No Injuries were sustained from this accident.

The mine experienced an elevated amount of carbon monoxide in the gob area of the long wall approximately 9,000 feet behind the active long wall face. After bag samples were taken and analyzed MSHA was notified.

Employee was trapped in T-2 Elevator traveling from repair bay to balling floor. He was trapped for approximately 70 minutes. MSHA was contacted at about 01:45 AM. No injuries occurred.

A roof fall occurred in the 4 right/4 left North Return side rooms above anchorage point of roof bolts in the first intersection at 0-150 of the #2 room. All approaches to the intersection were dangered off.

A rock fall was found between overcast on 2 West Mains in the #1 entry at mouth of the 2 left submains. Fall measured approximately 20' wide x 30' long x 5' thick.

A roof fall occurred in the J1 butt section in the #3 entry 75ft. inby survey station 2682. The fall measured 20ft in width and 95ft in length, it was approx 7ft in height. Fall was found on a pre shift exam. No men underground when cave was found. Entrance to cave were timbered and dangered off. Moved to location 300ft outby cave area to resume mining in J1 butt rooms.

At approx 1:15am the production hoist #30 lost power. Electrician were called and found and replaced a wire that powers hoist controls. At approx 8:30 am the hoist was put back in service. MSHA was called at 1:15am and hoist was reported down. At 9:20 am MSHA was contacted that the hoist was back in service.

An unplanned fall of the mine roof occurred on the tailgate entry of the 14 Left longwall at approximately 20+00 measuring 30' long x 8' wide x 4' high. The fall was not above the anchorage zone of the bolts and ventilation was not impaired but it prevented passage of men off the tail of the longwall face. The incident was immediately reported to MSHA and WVOMHST

While emptying pulverized coal from storage tanks as part of a kiln shutdown procedure, an excessive amount of coal dust was pumped to the precalcer, resulting in a significant puff. No injuries were sustained as a result of the incident. There was no equipment damage as a result of the incident.

A non-injury roof fall occurred at this mine 2 breaks inby the #3 intake portal. The fall was approximately 22'W X 20'L X 4'H. The area has been breakered off.

Miners setting pump in left ramp noticed roof fall had occurred sometime over Thanksgiving holiday in adjacent ramp. MSHA was immediately notified.

An unplanned roof fall has occurred at Short Sub Mains #5 entry spad 3067. The roof fall was approximately 30' x 40' x 4'. No injuries or equipment involved.

A rock fall was found at break #31-#32 on 5 belt on the 1 North Mains. Fall measured approximately 19' wide x 40' long x 3'-5' thick.

PM service truck was heading southwest on the haul road to the Ready Line and did not see haul truck turning right to enter on to the haul road to the Ready Line in the same direction. PM Service truck hit haul truck in the front driver's side tire, slide several feet sideways before being overturned and sliding 17 feet before coming to a stop. No major injuries were received.

The mine was idle, the outside communications employee noticed he lost power at the #7 mine. EE went in to see what happened and found a roof fall at x-cut #16 spad (8+90). The fall was 8' high x 20'long x 25'wide. EE called the fall in at 1:38 A.M., we then cribbed around the affected area and then started re cable bolting the affected area.

While testing the escape hoist at crosscut 66 the hoist would go to the bottom landing but when trying to bring hoist back to the top landing the control fuses would blow. After investigating the problem it was found that the mini processor was bad.

Roof fall was discovered in the #9 seam, 3rd North West Sub main, #6 Entry in the pillar line between spad numbers 2+10 and 2+80. The approximate size of the fall was length 50', width 18', and height approx. 18'.

A roof fall was found by a shift manager in our M3 Main Return Entry. Specifically x-cut 17 entry #3. The fall did not cause injury to miners, affect ventilation or impede passage of miners. It was found at 7:40am and the MSHA hot line was called at 7:45am.

THE EMPLOYEE WAS USING THE FREIGHT ELEVATOR IN THE PELLET PLANT WHEN THE DOORS OPENED PART WAY. THE EMPLOYEE WAS IN THE ELEVATOR FOR AROUND AN HOUR PRIOR TO SOMEONE ASSISTING HIM TO GET OUT.

While attempting to dump his load, a haul truck operator of a 100 class haul truck backed thru the safety berm causing the unit to settle in an upright position. As a result, the operator was unable to safely exit the truck and was removed with a man-basket. The berm was not of adequate height for this model/type unit.

A roof fall was discovered in #7 entry in the intake, it begins appx. 100 ft inby Spad 172...60ft long, 18-20 ft wide, 5-6 ft thick. Approval has been given to move the intake stopping line and primary escapeway to allow for the intake air course to bypass the fall. The inby end of the fall will be cleaned and supported according to the approved clean up plan.

An unintentional roof fall has occurred on the 3D submain at the 111-XC between the #3 and #4 entries. The fall was approximately 18 feet wide and 60 feet in length and over 10 feet high.

AT APPROX. 7:30 PM 11/20/11 A ROOF FALL WAS FOUND IN THE MAIN WEST RETURN AT X-CUT 88 ENTRY # 11. ALL APPROACHES TO THE FALL WERE PROPPED AND CRIBBED.

A haulage truck inadvertantly raised the bed into dump position in a low back area mashing the high voltage cable, thus causing a power loss to the 85-15 area of the mine and ths resulted in loss of power to the man cage. MSHA was notified and the mine was evacuated until power was restored to the man cage approximatley two hours later.

AT APPROX.9:45 PM ON 11/20/11 A ROOF FALL WAS FOUND IN THE MAIN WEST INTAKE BETWEEN X-CUT 70 AND 71 IN ENTRY # 9 ALL APPROACHES TO THE FALL WERE PROPPED AND CRIBBED.

Damage to the upright guide that keeps the cable from touching the ground was pulled loose and caused the cable to jump the bull wheel.

AT APPROX.10:20 AM A ROOF FALL WAS FOUND IN THE MAIN WEST INTAKE AT X-CUT 104 #7 ENTRY. ALL APPROACHES TO THE FALL WERE PROPPED AND CRIBBED. THE ORDER WAS LIFTED IN THIS AREA ON 11/21/11.

There was a roof fall @ 28 x-cut (20+60) 0n 2 belt #3 entry. The fall was approximately 18' wide x 20' long x 6' high.

The ABB hoist power convertor kicked out and the hoistman could not reset the device. An electrician came over to trouble shoot the problem and found that a relay had dirty contacts. After resetting the power several times, the power kicked in and the hoist was functional. No damage occured to the system.

The portal passenger elevator went down at approximately 7:05 AM. Schindler Elevator Service was called to evaluate the occurrence and correct the problem. The elevator was returned to service at approximately 10:30 AM. A defective door strut, that allows the elevator doors to shut, was found to be the problem.

A fall of the roof occurred on the 1st S.W. Sub Mains 4B x-cut 21 #3 entry primary escapeway. 19' length by 19' width by 5-6' height.

While making weekly examination of 002 section primary escape way a non-injury roof fall was found in the #2 entry 10' inby survey station 965. The roof measured approximately 60' long, 20' wide and 8' high.

In the middle of the shift the hoist pin broke on the I contact. This shut the hoist down creating an evacuation situation due to not having 2 escape ways to surface. Mine Evacuated. No injuries or other problems. Hoist pin was repaired and back in service at approximately 7:19 PM

Sperry Well Drilling Co. was drilling a well for a customer and drilled into the mine. Water entered the mine through the 6"" hole. The hole was plugged and grouted.

We experienced a rock burst on the 5900 mainline during our scheduled blasting time. The rock burst measured over a 2.8 Richter. The rock burst caused some damage in the 5900 54 Ramp as well.

A fall of roof above the anchorage of the 4ft resin roof bolts occurred in the #5 intake entry at 30c/c along the #2 belt. The fall was apx 20'lg X 20'wd X 6'thk. No one was injured during this event. The primary escapeway was blocked & all persons were evacuated outby the location until another primary escapeway could be established. The area was dangered & approaches cribbed.

A fall of roof occurred in the South East mains at cross cut # 30, # 1 entry. The fall measured approximately 100 feet long x 20 feet wide and the thickness is unknown. The area was supported with 6 foot fully grouted roof bolts.

A roof fall was observed in the #4 entry of the 2A Bleeder Panel. The roof fall is located in the intersection at survey station #999. The roof fall is approx. 20' Long X 30' Wide X 8' High.

An unintentional roof fall was discovered on the #3 Section, #5 Entry, Near Spad #2209. The fall measured approximately 25ft Long x 20ft Wide x 8-10ft High. No injuries resulted from this incident.

At 7:15 am on Nov 14 2011, the elevator became inoperable. After troubleshooting it was discovered that a door switch bracket has broken. The piece was repaired and the elevator was back in service within the hour.

The brake system on the hoist car at Roadfork # 51 mine malfunctioned for a period of time that exceeded 30 minutes. When examined it was found that a drum brake limit switch had failed. The switch was repaired which restored proper function of the braking system. No injuries were reported.

Roof fall occurred in the #8 intersection 1 Break outby spad 4066. All accessible entries leading into the fall area have been supported and dangered off, and will not be cleaned up.

A ROOF FALL was discovered during the weekly exam one break inby station 2636. The fall area was bolted with 42"" fully grouted rods with 8' cable bolts installed in the intersection on a five bolt pattern as supplemental support. The fall measured 20' wide x 40' in length & 8' in thickness. All entry points were timbered & dangered off to prevent access to fall area.

During an idle weekend examination a roof fall was discovered on our 002-0 MMU. The fall is located in the second open cross cut (xc-42) in number 3 entry. The fall was found at 11:07am and called into the MSHA Hotline at 11:17am. The fall did not cause injury to miners, impede passage of miners, or affect ventilation. The fall occurred in an intersection only.

while performing the daily checks on the Raise climber the upper motor became inoperable. The motor was replaced and the daily checks were completed.

While cutting towards the headgate with the longwall shearer, sparks from the cutting drum apparently ignited hydrocarbons coming from the roof resulting in a fire. The fire was extinguished in approximately 20 seconds by the fire suppression system and a water hose.

An unintentional roof fall was discovered in the #2 Entry, #3 Belt, Spad #2370. The fall measured approximately 80ft Long x 20ft Wide x 8ft High. No injuries resulted from this incident.

The relief vent on the #1 coal system was not rated properly for temperature. We believe that the teflon film in the vent failed. This caused the cyclone to lose its vacuum. Somehow the erratic operation allowed the coal to ignite.

An excessive amount of methane was detected at the X Seal area. The mine was evacuated and surface drilling was begun to alleviate pressure in the sealed area.

A fire occurred in the engine compartment of the Caterpillar D10 dozer as it was pushing coal on the raw coal stockpile. The fire was quickly extinguished (under 5 minutes) as the fire suppression system functioned as designed. (*No personal injuries. Both the Federal and State agencies were notified)

The hoist had been chaired to load a flatcar to be brought to the surface. When the hoist man went to un-chair, the system would not respond. Subsequent troubleshooting found the service hoist hydraulic power pack control card failed. The card was replaced and the hoist returned to service at 10:10 a.m..

At crosscut # 4 on the primary escapeway of the #3 portal a rockfall in the intersection occurred no one was in the mines at the time of the fall the fall was discovered by the foreman on his preshift no one was injured water and loose laminated shale was the cause of the fall

#3 hoist tripped due to an ems error. Hoist was reset, there were still several escape routes to surface for personnel underground.

At 4:30 pm a roof fall was discovered in the 3 Left/1 East section above the anchorage point of the bolts. This fall was in the old set of Return Rooms at 1+00 intersection of the #4 Room. This Room was turned off 'A' Entry at 28+07. This area is part of the 7 Day Examination.

Brush fire along the south and southwest sections of the quarry and buffer area. Brush along the freeway ignited by an unknown source. Fire traveled up the hill to the outside edge of the quarry and continued west into the buffer zone. Honolulu Fire Dept. contained the fire within 3 hours. HFD remained on site to monitor and extinguish hot spots. No miners were injured.

While performing a regular rope inspection using non-destructive testing, the hoist rope was found to be out of compliance. The rope was removed from service and replaced with a new rope.

In the Return, a slip had been cribbed in the intersection. The fall was left of the cribbed area, left of spad #19. State and Federal Agencies were notified. The entries to the fall was cribbed and dangered off.

A NON-INJURY ROOF FALL WAS DISCOVERED AT CROSS CUT 146 IN THE RETURN ON THE 3-SOUTH MAINS. THE FALL WAS IMMEDIATELY REPORTED TO MSHA. THE FALL IS IN THE #2 ENTRY APPROX. 20'WIDE X 20'LONG X 8'THICK.

A roof fall occurred in the 1st East Submain in the #6entry at spad #19+40 measuring approximately 20ft. long x 18-19ft wide x 8ft.high. This area was supported with 4ft. fully grouted roof bolts and 8ft. cable bolts.

Longwall panel 59 experienced adverse roof conditions at the headgate which impeded passage to the primary escapeway. The heavy roof set down on the headgate but there was no roof fall.

An unplanned roof fall was discovered by MSHA on 11/6/2011 during evaluation of a near by fall. The fall measured 19'x 60'x 6-8'high. The fall was in the 1st West Submain off of the 1st North submain @ 26+75 in Entry #7. The fall did not affect ventilation, passage of men or cause personal injury.

An unplanned roof fall was discovered by MSHA on 11/6/2011 during evaluation of a near by fall. The fall measured 19'x 40'x 6-8' high. The fall was in the 1st West Submain off of the 1st North Submain in Entry #6 @ station #29+95. The fall did not affect ventilation, passage of men or cause personal injury.

At approx. 6:00 A.M. on November 6, 2011 there was a hoist malfunction at the Coy mine, MSHA hot line was called and the incident was reported. Evacuation of the mine commenced through the Hicks Shaft, the problem was found and repaired. Employees were allowed to renter the mine and proceed to work by 9:00 A.M.

While contractors were removing damaged slope plate out of #5 product bin soda ash sloughed from an unknown location into bottom of bin. All personnel were evacuated with EE in the bin thigh high in material for several minutes and another EE assisted out right away w/another EE. 7000-1 submitted electronically on or about 11/30/11 but unable to locate submitted 7000-1.

The Lakeshore elevator, which is the secondary escapeway for all working sections of the mine, ceased operation for longer than 30 minutes.

EE went outside to get supplies. When he started back in the mine he found the roof fall at 3+25 on 1 belt x-cut 4-5. Fall was approximately 40' long x 18'wide x 6'-8' high.

An unintentional roof fall occurred @ 93+66 on the Main North (crosscut 118) in the #3 entry. The fall was in the entry. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 20'x 15' x 4' high and will not be cleaned up.

A ROOF FALL ABOVE ANCHORAGE @ 31 CROSSCUT OF THE 6B TAILGATE ENTRY. THE FALL IS IN THE CROSSCUT TO THE GOB. NO INJURIES; NOT AN ESCAPE WAY; TRAVEL & VENTILATION HAVE NOT BEEN EFFECTED. A BREAKER ROW OF POSTS HAVE BEEN INSTALLED IN THE CAN ROW.

On 11/04/2011 an unintentional roof fall was determined in the 3-right section between X-cut #5 and #6 in the #3 entry (belt line). The fall was approx. 20' long X 20' wide X 10' high.

A roof fall was discovered 2 breaks inby spad #33. This roof fall measured 6' thick by 30' long by 30' wide (extending into the crosscut on either side). This area was mined in May of 2006. The fall does not impede ventilation or travel and will not be cleaned up. The fall has been dangered off and cribbed.

A roof fall above the anchor zone occurred in the ""B"" entry of the West Lease continuous miner section (MMU 005-0). Attempt was being made to mine this entry to the surface and the surface area fell onto the head of the continuous miner. The miner became stuck but there were no injuries.

A roof fall occurred in the #3 entry of the 1st main north angle off the 2nd west, unit #6 MMU-067-0, spad #13+14, just inby last open cross cut, up to face. The fall was approx. 40'L x 18'W x 6'H. There was no equipment involved, no injuries and no disruptions to ventilation. The fall was discovered at 4:58am.

An unplanned roof fall occurred in the Roadfork #51 mine, 1 break inby spad #770 that measured approx. 30' L. X 17' W. X 6' H. No injuries were reported. The area was developed on 11-30-09

A roof fall occurred in the #3 entry of the 1st main north angle off the 2nd west, Unit #6 MMU-067-0, spad #23+14. The fall was approx. 20'L x 20'W x 12'H. There was no equipment involved, no injuries and no disruption to ventilation. The fall was discovered at 8:25 AM.

A non-injury roof fall accident occurred on active 003 & 004 mmu in the last open cross cut between #2 and #4 entries above the anchorage zone of 6 feet. The fall was approx 20 feet wide by 150 feet long and approx 7 feet thick.

Broken bolts on the main production hoist contactor would not allow the contact to close in the switch. Mine evacuated. No injuries or other problems. After troubleshooting to find problem, hoist released for service at 12:15am.

A roof fall was discovered in the 001 bleeder system in the 1st east submains. The fall did not affect persons, equipment nor ventilation. The fall was apx. 20'long X 18'wide X 6'high. The fall will be left and not cleaned up.

A roof fall was discovered on the old abandoned 002 section. The fall was apx. 24'long X 18'wide X 15-18'high located in the #6 entry 1 xcut inby spad 274. The fall did not affect persons, ventilation nor equipment. The fall will be left in place as it would be safer than to clean up.

A unplanned roof fall occurred in the #6 Entry at spad # 2322. Fall is 7,200' from active 001-0/002-0 section. Area was initially supported with 5' torque tension bolts. Fall meas. 20' L. X 20' W. X 6' H. Ventilation and passage of persons were not affected. Fall will not be rehabilitated. State and Federal agencies were notified immediately.

Coal in TK-102 was heating up the lower cone of the tank. Tank was emptied, water was used to cool the hot coal. Electric radiant heater was installed too close to the cone. Heater was temporarily removed and planned to be re-installed at a safer distance from cone.

A roof fall occurred in the #9 seam in the 1st West Sub-Main at crosscut #7 starting at edge of the pillar line between the #2 and #3 entries and extending half way into the pillar line between #3 and #4 entries. The fall measured approximately 80'x18'x10'. The area was supported with 6' tension rebar roof bolts and 8' double lock mega bolts.

A roof fall was found on 11/1/11 at xc 58 entry 4 in 8 LMS. Fall was in a four way intersection that had been mined in 10/2008. Fall was 20' W. X 20' L. X 10' H. Roof had been supported with 8' full resin bolts. No injury or property damage resulted. Fall will not be cleaned up.

Two miners working in an underground bin excavation were attempting to level the muckpile by blowing muck down a hole at the brow. They were both tied to retractable fall restraints when the muckpile slid down the repose angle unexpectedly. One employee manually pulled his line and engaged his restraint but the second man was engulfed and suffocated by the finely broken muck.

He was seen at the ER and released to return to work by the ER physician. Employee was setting the bolter up for the first row in 7 Right. He was at the controls on the operator's side when the rib turned loose and the rock fell on its edge catching his right foot. The rock was 69"" long, 24"" wide at the widest point and 6"" at the narrow end. It was 11.5"" thick at the thickest point and 6"" at the thinest. This resulted in multiple fractures in his foot.

Did not work 11/30/11 then came in 12/1/11 saying he saw a Dr. and was off work until he could have an MRI.

A brattice fell and hit the miner. It actually hurt his back, knee and thumb. Everything but the thumb got ok. The thumb had to have minor surgery. He missed two days for surgery and recoup and returned with no problems.

EE was lighting the natural gas pilot with a 16 ft extendable aluminum pole from outside the man-door to the kiln, which was common practice. Injured employee was standing behind him when the pilot was lit. The ignition of the pilot & subsequent back draft was larger that normal. EE turned away from the kiln and contacted the other employee, knocking them both down.

Individual was assisting in moving the Cat Fletcher Bolter. He grabbed onto a rib bolt while stamping on a piece of curtain which was stuck in the cats of the bolter. The bolter moved jerking the curtain out from under his feet causing him to fall. The right hand was holding the rib strap, cutting through the glove causing a laceration on right middle finger.

Employee was in confined area removing stacked cribs when he was struck by crib blocks that fell from the tailpiece. Employee immediately came out and was seen by doctor sustaining a contusion to his leg. Employee was ordered from doctor to remain off work for an uncertain period of time.

Excavator #2 mirrored movement of excavator #1 without direction and because it did not stop as it should have, it pulled the rail towards itself and struck the leg of employee.

The employee was changing scoop batteries in a crosscut on the Unit 4 travel way, when another employee was driving out battery-first on a ram car and struck the front of the scoop bucket, causing the scoop to roll backward. Injured was walking across parking lot to plant entrance during a snow storm and slipped on accumulating snow/ice in the area resulting in a fall to the ground. EE foot caught traction resulting in a jarring moment to his lower leg above the ankle. EE continued to work out his shift and sought treatment after hours when pain did not subside.

Employee was moving a 12""x12"" cribbing block when he twisted his left knee. The employee initially stated he desired no further medical care, however approximately two weeks post incident, the employee stated he need additional medical care for the injured knee. This report was filed due to the employee being prescribed a temporary knee brace.

Employee was replacing the idlers on the conveyor belt when he pinched his finger between the idler assembly and conveyor frame.

Employee was repositioning dumpster onto the forks of the forklift when the dumpster shifted pinching his left middle finger in-between the dumpster and the frame of the forklift. (Placing body parts in-between a stationary and moving piece of machinery.)

After working in an awkward position carried several 5 gallon buckets of oil to fill the tank and felt pain in his lower back. The pain worsened overnight and he completed a report of incident and sought medical attention the following morning. Placed on a lifting, pushing, pulling and bending restriction until seen again by his physician 11/23/2011.

Worker was ""barring down"" feed material in an attempt to clear a ""plug"" in the cone crusher. During the process, the worker injured his left, ""pinky"" finger (most likely when the worker's finger was ""pinched"" between the bar, and another object - cone, rock, etc). The worker was wearing gloves at the time of the incident.

Mechanic suffered a laceration to his Left thumb, on the dorsal side, while drilling a hole in plumbers strap to secure the air line seal air on R1 pump discharge bearing. The drill caught and the metal curled toward him and cut his Left thumb between the 2 joints.

Hoisting powder truck to put on the track to unload it. When hoisted up he pushed it over to the track and right shoulder popped when he went to unload it at powder mag and felt pain in right shoulder

Fall to the walkway or working surface

Employee was starting a pump during a rain event. He was walking back to the truck when his boot got stuck in the mud. When he pulled up on his boot to take the next step he lost his balance over the suction hose and fell. This caused the employee to suffer a sprained ankle.

Struck against stationary object

As Employee was walking he stepped on a rock which caused his right ankle to roll. Employee was treated at Med Express and xrays were taken.

Removing bend pulley from conveyor. Injured worker grabbed onto lifting strap that was hooked to the pulley that was hooked to crane. Pulley shifted and his left hand was caught between strap and beam. Left ring finger was lacerated, requiring 7 stitches.

Employee was picking up rocks under cone crusher feed belt.

Fall from ladders

EE was descending a vertical ladder when his foot slipped off a ladder rung. The ee had his hands on the outer frame of the ladder and was able to catch himself before hitting the ground. The ladder is approx 10 feet in height and the ee slid approx 5 feet before catching himself. The ladder is exposed to weather, and it was raining at the time of the incident.

Employee was going down the ladder on a piece of equipment when his wedding ring caught on the ladder and hyperextended his left 4th finger and left wrist.

While performing cleanup a mill operator slipped and fell to the ground.

Fall from machine

The employee was adding hydraulic oil to the dump truck when he slipped from where he was standing and fell. The fall resulted in him twisting his ankle. He thought it was a sprain, but when he went to the clinic to get checked they found his ankle was broken.

Employee was walking through the plant and stepped in a hole causing him to sprain his left ankle.

Injured employee slipped into a muddy ditch on the offside of the belt between 3 Head and Drive Motors causing him to sprain his right ankle. He was transported to the ER for evaluation and diagnosed with a sprain. He has been referred to orthopedics.
