Accident type, without injuries

A roof fall occurred in the 7 Mains section, # 3 entry at location 7 + 20. The fall was approximately 18 feet long x 16 feet wide x 4 feet high.

The DC contactors in the hoist room control system fused together and at the same time the dynamic brake control unit burned its' circuitry control boards. The boards and circuitry have been replaced and hoist is now running.

The spillway pipe of CP-1 pond which runs through the dam in a morning glory arrangement developed a leak around the pipe. As the flow around pipe increased it accelerated erosion making the leak larger. The end result was the CP-1 pond was drained into an adjacent wetland. The water was unimpacted water. The dam surface (roadway) did not show any subsidence.

At approximately 6:55 PM the service hoist seized to function. The mine was evacuated via the emergency hoist after the initial inspection did not reveal the problem. A piece of chain was found fouled in the bottom gate target/sensor that had apparently fallen while supplies were being transported.

At 8:38am the #3 hoist tripped out due to a drive fault alarm with shaft crew personnel on work deck.9:01 am evacuation initiated and MSHA notified.9:58am all UG employees out of the mine. Shaft crew employees lowered to mine landing at 10:35am. Rebooted TBR PLCs. Shaft Crew brought out in inspection speed. Shaft crew out of the mine at 11:25am.

The rack and pinion elevator on the west coal storage silos became disabled while two technicians were riding in it. They were in the elevator for more than 30 minutes and subsequently safely moved to the ground level. These people were never at risk of injury.

An unplanned roof fall occurred at the Road Fork 51 mine that measured approximately 20' long by 20' wide and 8 feet in height. No injuries reported. This area was developed in December 2009. The location of the fall was 1 break inby spad #975.

An unplanned roof fall occurred in Wyoming #2 mine at #6 break, along #3 belt, between #6 & #7 entry, between spad #708 & #721. Measurements were unobtainable due to this crosscut previously being cribbed off on both sides for bad roof conditions. No injuries were reported. The area was developed on 8/10.

hoist outage caused by blown fuse in substation behind hoist house. Miners were evacuated at 7:45 and reached the surface at approximately 8:00am. Power was restored to the hoist and the hoist was tested. Miners returned underground at 9:30am.

An unintentional roof fall occurred on 1st Southeast mains in the #1 return entry. The fall was at or near 800' outby the working section, and at or near 80' inby spad #2463. The fall was approximately 60' long x 19' wide x 10' high.

An unintentional roof fall was discovered during an inspection of the return air course. The fall was discovered in the South West Mains 1 Section, 2 breaks inby spad 1560 in the No. 2 entry. The fall was in the intersection of the No.2 entry, through the x-cut & into the No.3 entry intersection. The fall measured 188 ft long, 18 ft wide & 5 to 7 ft thick. Area developed 4/24/08.

The No. 1 Elevator was removed from service at 11:30 am. Schindler was performing a scheduled maintenance on the car safeties. The maintenance was completed, the car was tested, and returned to service at 3:45pm.

I called MSHA as soon as I found out there was a fire that lasted for more than 30 min. MSHA did send a rep out to survey the incident. I just found out that I had to report this on a 7000-1.

An unintentional roof fall occurred on the Unit 5 Entry 8 Crosscut 41. The roof fall measured approximately 18' in length, 17' in width, and 10' in thickness. A 103 (k) Order (8448265), was issued March 22, 2013, by MSHA Inspector.

(3-21-13)-(12:25pm) #31 hoist lost power. Electricians were called to check hoist (12:30) - called MSHA. Electrician found and replaced a burnt contact on one leg of starter disconnect. Contact was replaced and hoist was back on at (2:05)pm. (2:10pm)-called MSHA back to report hoist was ok.

While making return examination discovered 2 roof falls. #1 along 5 SW Mains in x-cut 4-5 about 20' long but was not affecting travel or air.

An unintentional roof fall was found while examining the travel ways on the 1st Southeast mains on 03/19/2013. The fall was at or near #2 (return) entry, spad #2427. The fall was approximately 18' long x 20' wide, 10' high.

An unintentional roof fall occurred on 3/19/2013 on the 1st Southeast Mains at the #5 entry, at or near spad #2425, 720' outby the face on 5A belt. The fall was approximately 36' long x 34' wide x 10' high.

A fall of roof occurred in the Southeast Submain at 95 crosscut, Entry # 5. The fall measured approximately 55 feet long x 25 feet wide x 15-18 feet thick. The area was supported with 6 foot fully grouted.

On March/16/2013 the (30) day hoist inspection was being conducted on the (19 man) hoist at the Affinity mine. During the inspection it was observed that the slack rope brake was not adequate to the manufactures specifications. Both regulatory agencies were contacted, and the mine personnel was evacuated from the mine by 11:00am. The hoist was operating again at 1:15pm.

The brass bearing on the lilly limit ceased up causing the cast iron bracket to break, the bearing and bracket have to be made at a machine shop. So the hoist is inoperable.

At 9:45 #3 hoist displayed encoder fault causing hoist to trip out.10:00pm MSHA notified. Co. contacted to troubleshoot issue.Hoist reset,test trips ran,hoist functioned properly,decision made to bring crews out via #3 hoist.All crews out of mine @ 11:48pm.MSHA notified apx.11:55pm. Fault caused by loose connection in the PLC rack.Connection secured, multiple test trips were ran.

Around 0605 #3 hoist kicked out at 200' from the collar. Reset hoist brought crew to surface. MSHA Notified 0614, began evac out #1 hoist. All crews out by 0730. The set screw of the encoder on the hoist drum shaft had become loose. Set screw was tightened, the system reset, and test trips ran. All personnel allowed to return to the mine and MSHA notified at 0800.

A roof fall was discovered by an examiner in a return entry of the West Mains 1 Panel, one block inby spad 0579. There was no interruption in the ventilation and no equipment of value involved.

A rock fall occurred on 3/11/13 in #5 entry approx. 70ft long x 19ft wide x 8ft thick in the 2nd West Panel off the 1st North Panel at 14+00. The fall was cribbed around.

One of the trucks being used for the overburden stripping activities caught on fire, probably due to an unknown damaged hydraulic line. The operator heard a pop, then saw that the fire had started. The operator of the truck was unable to extinguish the fire, so the engine compartment and cab completely burned.

An unplanned roof fall occurred at the road Fork #51 mine that measured approx.15' L. X 6' H.and 20' W. No injuries were reported. The area was developed in November 2009. (See attached Map)

The ceiling collapsed in L-10 and was witnessed by 2 miners in K-10. No one was injured and supervisor bermed off K & L until MSHA arrived.

The fan attendant observed smoke coming out of Orndoff Shaft during the normal daily inspection. Mine Management was notified and although no flames were observed the mine was immediately evacuated. Individuals began investigating the area where the smoke was believed to be coming from and also evacuated when they encountered smoke.

Service hoist became inoperative. A positioning wire was damaged at the surface area of the hoist making the hoist incapable of landing in the required location on both the surface and the mine bottom. All mine personnel was withdrawn from the mine and both regulatory agencies was contacted in an timely manner. The hoist was back in full operating capacity at 9:40am.

Fall was in the intersection of #0 Entry at Sta #4930 (100' inby). Area was bolted with 6' full grouted supertwist bolts 3/4"" and 12' cable bolts.

The contactor on Number 2 Drive of the Production Hoist went bad at 1:00 p.m. It had to be replaced. This caused the Production Hoist not to operate. It was replaced by 3:00 p.m. The Production Hoist and Shaft is the second escape way out of the mine. The mine had to be evacuated until the hoist was repaired.

Rock fall was discovered on fire boss run by mine foreman. The fall was in #4 face extending across the face through intersection to halfway between 5 + 6 headings.

An unintentional roof fall occurred on the 1 North Mains section, MMU 067-0. The fall is located in the #3 entry between 79 and 80 block. the fall measures approximately 16' wide x 40' long x 7-8 high.

4:30pm we received an alarm that the East&West explosion vents on the #1 coal mill had opened. No personnel injuries or property damages. It was discovered that an accumulation of smoldering coal dust was foiund at the bottom of the mill. This was determined to be the source of the ignition. All parameters of the coal mill circuit were normal leading up to the vent doors opening.

At 0736 08 March 2013 the Interim Mine Manager checked the shot that the 3rd shift supervisor had conveyed interest in the mine manager reviewing and noticed some bolts on the ground that had come down with the shot. It was later determined that this was not reportable by the mine however the field office supervisor stated that he thinks it is and directed us to submit a 7000-1.

The mine hoisting system was inoperative due to electrical problems on the surface. The problem was diagnosed and the hoisting system was operating again at 7:30pm

Employee was elevator attendant for the day. When the elevator just stopped for no reason. he attempted to re-engage, but it just would not engage. So he picked-up the phone and called the control room. Did not inform them that he was in side the elevator. The control room operator called for service. So the service personnel did not place the call on priority mood.

Secondary Escape Hoist was found frozen at 9:55 am and about 11:07 am it was unfrozen and working properly.

A rock fall occurred on the high wall at the #1 (39A punch out fan). The fall of the high wall slid behind the fan portal canopy blocking the entry that was the intake Escapeway for the 39A Right Mains. Size of the rock slide was approximately 50ft wide 25ft high.

An unintentional roof fall occurred on the Main West 2, Entry 1 Crosscut 83. The roof fall measured approximately 20' in length, 18' in width, and 10' in thickness. A 103 (k) Order (8448262), was issued March 7, 2013, by MSHA Inspector.

A fall of roof occurred in the 4th right panel off the 3rd SW Submain in entry # 11, crosscut 6-7. The fall measured approximately 80 feet long x 20 feet wide x 9 feet thick. The area was supported with 5 foot fully grouted roof bolts.

Employee was operating 740B Caterpillar Truck. The truck engine started to regening, so the operator went to lunch in the building, when he came back from lunch the truck engine was on fire. the fire burnt engine and cab on truck. It took about 25 minutes to put out the fire on the truck. there was no injury to any person.

The voltage regulation transformer for the production hoist control circuit had and internal problem that would not allow it to provide full voltage to the system & a brake solenoid coil connector shorted internally and needed replaced.Both issues were evaluated and corrected.

Employee was operating a loader near the highwall when a shot was initiated.

The Continuous Miner had started into a left hand turn on the second lift off the pillar. A roof fall approx. 22'x25' ranging in thickness from 12"" to 22"" occurred, striking the employee, causing fatal crushing injuries.

EE was fatally injured while installing a rib bolt in the 8 Right Crosscut on Unit 1 when a slick-n-side formation fell from the mine roof inby the ATRS of the bolt machine, struck the mine floor and protruded into the employees work area. The employee was struck against the back with the rock and caught between the drill pot/mast area of the Fletcher DDR-13 WT he was operating.

Employee riding in a mantrip with his hand on the edge of the fender and as the mantrip was backing up it hit on offside in the rib catching his hand between the mantrip and rib severing his fingers.

As employee was moving a centralizer for a drill with a chain using an overhead crane when he grabbed the chain to steady the load and the chain turned.

Employee was helping the men change and remove screens for the Telesmith S-2 Screen, when he pulled something in his lower abdomen.

IE's tip of left index finger was smashed during a repair on a hammer drill. After taken to the emergency room, at approx 3PM IE's finger was removed back to just below the finger nail. IE returned to work on next scheduled day 3/10/13.

Struck by falling object

A bead flange from a haul truck tire fell from tire being installed landing on the left foot of employee breaking the employee's foot in multiple places. Employee required surgery.

Employee placed fingers in line of fire while attempting to keep spacer aligned in a hydraulic cylinder. As another employee pushed pin through cylinder, tips of left hand middle finger and ring finger were pinched off.

EMPLOYEE WAS HOLDING 7 RODS WHEN THEY FELL OUT OF HIS HANDS. THIS ACTION CAUSED THE EMPLOYEE TO LOSE HIS BALANCE AND FALL TO THE GROUND ON HIS BACK.

Injured worker was disassembling concrete forms on the work deck. He loosened the turnbuckle with a crescent wrench and over-exerted himself. On 04/18/2013, the doctor ordered him to be off and return on 04/24/2013.

Employee was cleaning up a large amount of spilled dust, sweeping and shoveling approximately 4000 lbs. Noticed a pain in her left hip. Not better on Tuesday, April 2 (her next reporting day). Went to Doctor, diagnosed as lumbar strain.

Employee states he was shoveling off tracks and couldn't finish, developed major back pain.

While assisting another employee in moving a ventilation slider tube, the other employee dropped their end of the tube. The injured employee complained of a pain in his low back. This event became reportable when the employee began missing work on April 3, 2013.

Employee was helping remove a hydraulic cylinder from a storage rack and as he reached to remove a block of wood from in front of the cylinder, the cylinder shifted & slid on the forks of the forklift and struck him behind the left knee causing laceration & bruising.

Employee was tasked with loading a 22lb pump into a man basket for transportation to an underground heading. Employee lifted pump alone, and sustained an injury to her back. Employee was diagnosed with acute lumbar back sprain, she received x-rays and an office visit. Employee to have follow up on 4-09-13 and may be tasked with light duty on 4-10-13.

Stated: Employee was walking from shower to locker at the end of his shift. He slipped on the wet floor and twisted his left knee. Sought med. treatment and released to RTW full duty. Results from MRI received on 04/09/2013 indicated degenerative changes and a torn meniscus.

After arriving on section employee checked face to see if any bolting was needed. After checking face then was walking back to belt set-up when fall occurred.

EE was unloading a bucket of miner bits from the top of a scoop when he complained of pain in his right shoulder. He was taken to Hospital where x-rays were taken and the physician recommended he take 2 weeks off and follow up with his doctor on 4/1/13. Location 6 ti 7 c.c. - spad 3002

Employee was leaving carpenter shop on bicycle traveling short distance to lunch room for afternoon snack prior to end of work shift. After traveling a short time, front tire of bicycle hit loose gravel. The front wheel turned abruptly, resulting in employee falling to his right side. In the process of attempting to break fall, he twisted right knee.

He and other crew members were moving I-Beams from a crosscut to the end of the track. As they were dropping the second beam onto the ground it bounced off the mine floor and struck him on top of his right foot causing two fractures.

Employee stepped off of deck of tractor he was operating and slipped, twisting his left foot and ankle, causing sprain/strain. Employee did not start missing time from work until April 2, 2012.

The employee was riding on golf cart and the golf cart became high centered on rock dust. The employee got off the golf cart to assess the situation and stepped in a hole and rolled his ankle.

EE (mechanic) was changing the left track on the D-10R dozer until #L1-1472 when the chain he was picking up the track up slipped and fell on his right foot. He drove himself to ARH in Hazard and received x-rays no other treatment. His right foot was bruised.

Employee was helping take apart a yellow discharge pipe when a rib rolled out hitting employee in the legs.

While traveling in a mantrip, the employee became airborne when the mantrip struck a bump. This resulted in a fractured left femur.

Employee was pulling on a wrench when he felt pain to his upper right arm.

Employee was doing routine crusher maintenance when he noticed a bolt was loose on the underside of the crusher. As he was walking to tighten the bolt he was looking up and tripped over a frozen clump of mud.

Struck against a moving object

Employee was operating Caterpillar 665 challenger tractor with a box scraper attached to the back of it. When he backed up to unload the box scraper he ran over a rock that caused the machine to jerk sideways causing his head to jerk to the side. Doctor wants MRI before returning to duty.

Over-exertion in pulling or pushing objects

Employee was pulling miner cable and water line when he felt pain in his right groin area. Diagnosed with hernia.

Employee was attempting to disconnect the water hose from the section feeder, he was unaware that the water hose was currently under pressure from the main water line. While disconnecting the hose, he was struck with the hose causing pain to his left shoulder. He started missing time on Wednesday 3/27/2013.

This report is being filed under protest and solely to abate citation 9005624. According to MSHA, employee incurred a reportable injury when he was carrying a bag of rock dust at 3A drive and had pain in his back. Brody denies that it breached and duty to report an occupational injury and reserves the right to contest the issuance of the citation.

Employee was hanging a curtain, tripped on the curtain and fell causing pain in his left knee. Employee continued to work until May 30th, 2013 at which time he began missing work and was scheduled for arthroscopic surgery on Monday, June 3, 2013

Miner was inspecting bearing on conveyor when he lost his footing, tripped and fell on the ground on his left flank/hip area and received bruises

Over-exertion in lifting objects

Employee was pulling the overshot & core tube out of the rods. He had the overshot supported with his right hand extended & raised over his head. The overshot came unlatched which caused his weight to shift all to his right arm and stress his shoulder resulting in his injury.

Employee was walking at the face area in mud that is ankle deep, got his right foot stuck in the mud causing him to fall. Causing a strain on his ankle. PPE was a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency was a factor.

Employee was tapping drive gear into a tram shaft with a crib tie and felt pain in his right arm and shoulded. Had MRI and was diagnosed with a torn rotor cuff and torn bicep muscle.

Employee picked up a drive shaft out of a D-9, he lifted it up about 2 feet and said he hurt his neck and shoulder.

Employee was changing a motor on the 2000 gallon fuel tank. As he was climbing onto the tank, he put left foot on 1st step and as his weight went onto left foot he heard a pop in his left knee. *Employee worked full duty until surgery on 6-10-13*

Employee was applying rock dust and cleaning the area when he tripped over loose material causing him to fall and injure his left knee. He finished his shift but went to the ER that evening. He was diagnosed with a Left knee injury and calf strain and has been excused from work.

IE stated that while walking in the #5 Heading of 1 North Mains he stepped in a water filled hole and twisted his left knee and ankle. The EE sustained strained ligaments in his left foot. This incident was not deemed reportable until 4/18/13.

Employee was tightening a bolt on the rear end of a scoop using a 3/4 inch drive socket when he felt a ""pop"" in his back.

Employee was assisting in the transportation of an injured co-worker. The responders had positioned the injured co-worker on a back board and placed the back board upon a battery powered mantrip. During transportation the back board shifted and pinched the assisting employees hand between the mantrip and the back board.

Employee and a fellow was cleaning a tailpiece with a water hose. EE jumped up on the tailpiece to keep from getting wet. When he jumped off the tailpiece he felt a pop in his ankle. He felt pain but continued to work and finished the shift.

Employee was mounting the walk-thru roof bolt machine and as he stepped up onto the back of the bolter he struck his head on the thread end of a torque tension bolt that was sticking down from the top. This caused him to strain his neck.

Employee stated he was descending the steps on the drill when his foot slipped off a step and he grabbed the hand rail to keep from falling. He sprained his lower back. Employee said he had three points of contact and his boots was muddy because it had been raining.

On 03/23/2013 at about 0200 am an approximately 60 lb boulder struck employee on left elbow after apparently coming off of a conveyor. Informed on 3/9/2013 that miner did not come back to work after his days off stating he hurt his back and hips the previous work days. While driving LHD miner ran over a rock and it jarred him causing pain in his back and hips.

Employee was sitting down under a belt and grabbed hold of the structure to stand up and his left knee popped and he fell to the ground. An MRI revealed on 4/15/2013 that the employee had a tear of the medial cartilage or meniscus in his left knee and is off work and is now a lost time injury.

Employee was working on construction of an overcast when part of the wall fell causing the constructed portion of the overcast to fall injuring employees left knee. Employee was diagnosed with a torn meniscus on 3-18-12 and became a lost time accident at that time.

The miner was driving a haul truck over a rut causing the seat to raise up and the force caused the nut holding the seat belt in place to come loose. She immediately stopped, got help and fixed the seat belt. Then told us her shoulder was hurt.

Employee was helping with track rail recovery, as rail was being pulled outby by a diesel locomotive. As the third rail was passing the switch tie the employee attempted to lift the leading end of the rail over the tie with a bar. As one employee was lowering the outriggers on the ES1514, the other employee decided to grab the come-along that was on the bed of the truck. As he did so, the left rear outrigger came down on the employee's left foot, breaking his left big toe, requiring surgery. Currently ONGOING.

Employee was working in the Jamaica Building preparing to clean belt hopper. While descending stairs, he stepped into wet bauxite and slipped, hurting his right shoulder. He returned to work with no restrictions. Pain continued to worsen and on 4/8/13, he was diagnosed with a torn rotator cuff (right arm)and returned to work with restrictions.

EE was cleaning rust off tank using a 7lb. grinder, complained about shoulder hurting next day to relief supervisor. On Monday 4/1/2013 ee complained to regular supervisor who then took him to the clinic. Modified duty was prescribed. While bolting a face cut. The bolter operator was in the process of preparing to drill the hole for the inside bolt on his side of the bolter. When the bit touched the roof, a piece of roof rock 1-2"" thick, 2' wide and 5' long fell from the roof. It hit the support arm of the ATRS and slid down it, hitting the bolter operator on the right side of his head, shoulder and chest.

Two employees were in the process of making welding repairs to the back of a skid steer loader bucket. The hoist cylinder continued lowering until it free fell forward and downward. When the employee realized the bucket was falling, he moved backward and the attachment ear on the right side of the bucket landed on top of his right foot crushing three toes.

Was using sledgehammer, felt strain in his back. Has been seeing Occ Med, but no medical treatment was prescribed. Because of continuing random pain, and previous laminectomy from home injury, he requested MRI which was read 4/22/13. Assessment lumbar strain, will now receive steroid injections. He continues to work on his regular job, but now with some restrictions.

Two employees were working between two buildings. While facing one building Ice slid off the roof behind and struck one of the employees on the back of right shoulder. The uninjured employee stated they had looked at the roof edge and saw no overhanging ice or water dripping and they made the determination that the area was safe to work in.

The employee was working on a booster pump. While pulling on a six inch pipe trying to get it into place, the employee felt a pop and tearing sensation in his left shoulder. This shoulder had been injured previously in 2010 that required surgery.

On 7/10/13, he went back to restricted duty & remains on restricted duty @ time of submission of E-doc 1802792. To date, missed work days are 3/7/13, 3/8/13, 10/3/13, 10/4/13.

Pipe was stopped up and the employees arm was in the pipe trying to clean to remove gob. When the gob came lose the employees arm was in the way causing the gob to break his right wrist.

Employee was tightening the main shaft nut when the tightening device bounced off of the knocker wrench and his right hand ring and little finger were pinched against the main shaft nut, requiring stitches to both, and a break on the tip of the little finger.

Employee was walking on ground level to stairs to 43 belt drive house. He grabbed the railing, slipped on snow and his right leg struck the angle iron of stairs and then went out from under him. He fell to the ground. He dislocated and fractured his right ankle.

Employee was hammering on tooth on a machine bucket. Chip came off and entered his stomach.

Employee states that while cleaning up, he was wheeling a wheel barrow full of bentonite to the front end loader to empty it into the bucket. He set it down and re-adjusted his grip on the handles. He lifted it up and as he went to push it over to dump, he heard a pop in the left shoulder area. Took to Urgent Care.

Two electricians were moving a section of dragline trail cable when it became snagged on a stump. The stump broke free causing the trail cable to sling forward striking the right ankle of the electrician working on the ground. He was diagnosed with a sprained right ankle at the emergency room.

EE reported falling on 29Mar13 but did not seek medical attention until 22Apr13. EE was opening a door to a storage shed when wasps flew towards him. As he was stepping backwards to get away from the wasps, he tripped over some pallets and fell to the ground. EE was diagnosed on 22Apr13 with a contusion to the right shoulder and released to work with restrictions.

Employee was climbing a ladder to top floor to check the position of a Clarkson valve when he struck his left hand ring finger on a metal plate welded to a piece of pipe next to the ladder structure. His finger began to swell and cause pain. Finger is fractured. Employee's work duties restricted on 3-25-13.

Injured was opening manual overhead door (chain-operated) when he felt pain in his right elbow. Injured requested and was provided medical treatment on 3/23, but wasn't given work restrictions until 4/9 making the injury MSHA-reportable.

While walking toward water truck, EE tripped over a pile of concrete and fell on his left knee and hand.

Employee was knocking grit off rails to install rubber components for a bucket. A piece of grit went into employee's right eye. Employee did not inform operator of injury until employee went to hospital to have grit removed from eye when eye became irritated. Employee suffered corneal abrasions.

Injured was helping to install a new 30"" conveyor belt at the HiCal crushing plant. Injured had put clamps on the belt in order to pull the belts together when one side hung up on part of the frame of the conveyor. Injured lifted up on the chain that was hooked to the clamp and pinched his right hand between the clamp and the angle on the conveyor.

While operating D-8 dozer, moving in reverse, dozer dropped unexpectedly on uneven terrain, jolting injured worker. Injured worker felt pain in lower back.

He was evaluated by a Physician on 3-20-13 and given restrictions and returned to work.

Employee was operating the man lift in order to remove snow from the conveyor belts. As he was elevating the man lift up under a conveyor, the machine cover on the man lift came too close to an ""I-Beam"" and his hand got caught between the control panel and the underside of the I-Beam. He lacerated his hand requiring 5 stitches and has hairline fractures on 2 fingers. Right Hand.

Employee slipped and fell while working on replacing steel beams. Fall arrest equipment consisting of full-body harness, twin-leg lanyard and horizontal ratline arrested the fall, but his right arm bumped a piece of angle iron close to his work, causing a mild sprain to his right shoulder area.

Employee was bending down and twisting to step down from an 18 wheeler trailer when he felt his left knee pop. This caused him to lose his balance, falling to the ground, landing on his feet. At that point, his knee gave out on him. He was diagnosed with lateral meniscus sprain and on 3/22/13 his duties were restricted.

While installing conduit, a fabricator fell approximately 6ft when the board he was standing on shifted. While the fabricator initially reported no injury, the next day he felt soreness in his back and was sent to the occupational health clinic. He was found to have severely pulled some muscles in his lower back. He was subsequently placed on light duty and returned to work.

Employee stepped back to observe ground conditions while assisting another miner who was bolting the roof with a jackleg drill. He tripped on the one inch jackleg air supply hose and fell backwards resulting in the back of his head striking the rib behind him. He received 2 lacerations to his scalp (treated with 17 staples) from a ground support plate installed in the lower rib.

Employee was carrying a 48 inch pipe wrench with his right hand and a 15# hammer on his left shoulder. EE started to bring down the hammer and lost control and the hammer fell onto his right hand that was holding the pipe wrench. EE's has a fracture metacarpal on his right hand. EE has been put on light duty and his next doctors appointment is in 10 days.

The EE was bending a 6 ft. roof bolt and twisted his left knee. There were no lost or restricted days at the time of the injury. This injury became worse after time and the EE required surgery on 05/01/2013.

Finger got caught between leverage bar and railing when EE was changing out a valve gate on a silo. Employee was injured on 3/11/13, but sought medical care on 3/25/13.

While bolting in the cross cut R1-R2. After inserting the bolt into the chuck. The operator accidentally bumped the rotation lever. This action caught the glove on his right hand to be caught in the rotation. This caused a fractured left 5th proximal phalanx.

Employee was changing out a header and was loosening the nuts on a Vic clamp and felt a pop in his left shoulder, which resulted in a sprain of the shoulder.

After the Helper unlatched the overshot and began lowering it to employee, the overshot slipped out of the helper' s hand and landed on the employee's right foot.

Employee was walking on top of a cover rail car to position a loading spout in preparation to load the car. Employee struck his head on a low clearance walkway above the rail car. The employee suffered strained neck muscles as a result.

EE was tightening up bolts on haul truck when he felt pain in his stomach. He was diagnosed on March 15th with a hernia and had surgery to repair the hernia the same day. The employee's next scheduled worked day is March 21st.

A Mill Maintenance Mechanic was assisting with the removal of a plow tip from a Raymond Mill. The plow tip was rigged using a chain and was being hoisted from the ground. The injured employee was guiding the plow using his right hand to hold the chain. The load shifted causing the chain to move downward, pinching the mechanics fifth digit on the right hand.

EE was using a fire hose to move the bird balls away from the work area so the Guzzler could remove the sludge from the pond bottom. He had been working from the horizontal spillway but had to reposition himself to a sloped section of the pond liner to do his task. While moving the hose he slipped and tried to break his fall with his R hand. Fx hand, but released with restrictions.

EE was climbing up on CH-368 to make repairs. Lost his balance and fell off. Twisted his right ankle.

Associate was plowing snow with the skidster, hit a frozen piece of material. Causing the associates head to jerk, which resulted in a sprain of the neck. The associate has returned to his normal work duties. Associate went in for follow up and needs some massage sessions to work out knots in his neck this was discovered on 3-19-13.

While replacing the blow bars in an impact crusher's chamber, the workers' finger was cut while trying to hold the nut with his bare fingers, while using an impact wrench to torque the bolt. His finger was cut by the spinning nut.

Lab tech was maneuvering a wheelbarrow filled with wet aggregate by backing through a doorway when his right forearm struck the door frame, resulting in a laceration requiring sutures to close.

Employee felt his knee pop while walking up to the MCC at the scrubber. When EE returned to his truck and sat down he felt his knee pop again but louder and accompanied by sharp pain. The pain did not recede so he drove up to the office. His knee was tender and when he tried to leave the office he could not walk due to the pain. He was then taken to the clinic.

While driving a haul truck in the ramp the employee hit a rough spot in the roadway that jarred him upward and backward in the seat causing pain between his shoulders.

EE was removing paper on pulley, belts running. Belt caught glove pulling it between 2 pulleys causing a laceration & hair line fracture to left hand 2nd finger tip. Required sutures & restrictions of limited use of left hand until 3-4-13 then, keep dressing clean & dry.

The operator was removing a guard on the bottom of the belt, the guard slipped and fell on his hand causing a laceration on the middle finger of the left hand.

Halfway through the lift, they lost momentum and sector struck the tub. Manager's third finger, right hand was pinched between the sector and pipe on edge of tub to hold guard. He received 10 stitches.

Employee was hooking rigging to a cat walk for removal, and stepped in a hole made by the disassembly process. In doing so, he contacted the front edge of the hole with his shin. This cut his leg, which required stitches.

While taking a scoop out of the mine, it came in contact with the rib due to wet roads. Caused a rib bolt to break sending a truss cable into the cab striking the operator in the back of his head. He became disoriented for a second. Diagnosis: Scalp contusion & abrasion, received a tetanus shot and released to full duty from Reliance Medical Group.

A piece of dust from the conveyor blew into her left eye. Potassium Permanganate is damaging to eye tissue on contact. Employee had on Company issued safety glasses. Employee utilized the new Eye Wash Station. Employee was taken to the South Texas Regional Hospital. Treatment was continued flushing of the eye and given antibiotics for her eye.

Checking muck pump without relieving pressure and lid of tank blew off striking him in the chest and laceratiNG his left thumb. The cut took four sutures to close. He returned to work his next regularly scheduled shift. THIS IS A MEDICAL TREATMENT INJURY. NO TIME WAS MISSED.

Injured was building a platform when the electrical hand held grinder he was operating kicked-back striking him on the left side of the face and chipping a tooth.

Struck by flying object

Employee was grinding in a confined space for most of his shift. At the end of the shift he felt a discomfort in his eye before he left the mine site. He did not report this incident until the following day. The hospital could not remove the embedded object and it was removed by an Eye Doctor on 4/1/2013.

Struck by flying object

Employee was assisting in replacement of a bulldozer ripper tooth when a metal fragment from keeper pin chipped off and struck him in the left forearm. Treatment for this injury included an incision requiring stitches.

Fall to the walkway or working surface

Officer was leaning over to pick up something while sitting on a chair and the chair rolled out from under her causing her to fall on the floor and hurt her wrist.

Employee was setting a bolt hanger when he began to spin the bolt the return of resin coming out of the hole flung off the plate and went around his safety glasses and into his left eye. It was believed this was filed on 4/4/13. It does not appear on the DRS so it is being submitted now. We have a printed copy on file from 4/4/14.

EE was wearing all PPE, when flipping welding hood down particle went into right eye. Could not was particle out with eye wash. EE went to urgent care .

Struck against stationary object

Employee was underneath equipment greasing drive lines on a creeper. He rolled out from under truck and hit his head on the guide for the garage door. He was taken into the hospital and received stitches.

Pinched L index finger while erection of steel frame.

Employee was using a hammer drill to drill refractory. Drill caught and spun out of operators hand striking him in the cheek causing a small laceration. Employee received stitches. Laceration occurred at the end of shift, employee returned next day to work.

Struck by falling object

Employee was operating a shuttle car going around a corner when the canopy of the Shuttle Car hit the corner and knocked down the brow which hit the employee's left thumb resulting in a fracture.

Fall from machine

Subject was standing on fuel tank of a D11 dozer. He slipped and jumped to the ground.

Absorption of radiations, caustics, toxic and noxious substances

Employee operating forklift on access road and was stung by a bee.
