Accident type, without injuries

At 1:55 pm on 3/31/2014, the door on the elevator at #9 Portal would not latch while on bottom. No employees were trapped at the time of the incident. The elevator was up and running at 4:30 pm.

While conducting the daily hoist inspection, the hoist safety supervisor system failed, resulting in the hoist being out of service for more than 30 minutes. The hoist was returned to service at approximately 12:20 p.m..

While lowering supplies with the internal hoist the internal hoist quit operating, which would not allow the service hoist to be raised due to the supplies still be connected to the internal hoist. All underground crews were evacuated and no persons were in danger.

A non-injury accident (ignition) occurred at this operation on 3/31/2014 at approximately 1320 hours. This happened on 7 section (MMU-035-0) inby the loading point.

Last shift Fri night noise was heard exiting the mine. Found lower NE wear plate folded. Plans made to repair the wear plate and inspect the shaft before the weekend crew came in. Sat morning, wear plate replaced, shaft inspected and discovered damaged guide at -965. Call to MSHA at 11:25am to report shaft down. Repairs completed and weekend crews returned to work at 4:00 pm

Water got into the slope car control box causing the PLC to fault. Fault in the PLC caused the brakes to lock up on the car while hoisting.

ROOF FALL - Occurred on the No. 6 section in the No. 3 and No. 4 entries. The fall fell out in the #3 entry intersection and through the yield pillar cross into the #4 entry. The fall was approx. 60 feet wide and approx. 30 feet long and at least 8 feet high.

During inspection of the elevator hoist a crack was found on one of the brakes shoes. System was immediately taken out of service. Repairs were made and returned to service on 3/31/2014 1:20pm.

An unintentional roof fall occurred on the 2 North 2, Crosscut 17 Secondary Escapeway. The roof fall measured approximately 10' in length, 15' in width, and 8' in thickness. A 103(k) Order (8450330) was issued March 28, 2014, by MSHA Inspector.

While inspecting the east production hoist motor, damaged bolts were identified between the coupler and the disc. Hoisting was stopped a mine evac was called and the damaged bolts were replaced.

Brake system was not functioning properly, was taken out of service until repaired and passed inspection. No persons affected. Was fully operational at approximately 1130am on 3-27-14.

ROOF FALL - Occurred on the No. 6 section in the No. 1 and No. 2 entries. The fall fell out in the #2 entry intersection and through the yield pillar cross into the #1 entry. The fall was approx. 60 feet wide and approx. 22 feet long and at least 7 feet high.

While operating a backhoe, employee struck a small inside berm and rolled the backhoe onto its side. Employee was wearing seat belt and sustained no other injuries but did have a brief period where he didn't appear to be completely conscious. Employee stated that he did not recall driving onto the inside berm and was reported as being groggy by the Emergency responders.

Roof fall on INE mains #2 entry 65 foot inby SS#13026 in the crosscut 73 intersection.

The mine examiner and an MSHA Inspector were traveling outby areas and discovered the fall, which was approximately 50' long by 20' wide and 3.5' thick. The area was timbereed off afterward. This area was mined approximately 30 years ago.

A non injury roof fall occurred above the anchorage zone of the 5 foot fully grouted torque tension bolt used for primary roof support in the 3-62 panel in the #7 return entry 3 - X-cut inby spad # 54682. The fall was 20 foot long, 19 foot wide, and 5.5 foot high. It was timber and danger off and will not be clean up.

Electrical Breaker for the Koepe air compressor tripped out multiple times and eventually needed to be replaced. Mine evacuated with no injuries and no issues. Returned to regular schedule at 6:20 AM.

A roof fall measuring approximately 18'W X 20'L X 10'H was discovered in the intersection of the #6 entry at spad 1216 in 2nd right. The roof fall was discovered during the weekly air course run & did not impede ventilation or escape ways.

The crabapple elevator was not operational for an extended period of time. As the elevator approached the top landing, vibration was detected. It was investigated and found that the Main elevator drive needed to be replaced.

An unseen roof condition above test hole height 150 feet inby spad number 73124 in entry number 5. The fall was 22 feet in length 19 feet wide and 8 feet high.

Roof Fall happened at 6 East #4 Panel (Intersections & Crosscut #3 to #4 Entry). Size of Roof Fall: 100' Long x 19' Wide x 6' High. Ventilation was not disrupted. Belt structure and High Voltage cable involved.

An unplanned roof fall occurred in the 1L mining section at 38 X-cut. The fall measured approx. 32 ft long by 10ft high by 26ft wide. The fall occurred in the return intersection. There were no injuries or equipment damage as a result of this event

An unintentional Roof Fall has occurred on the Old #1 Section #2 Entry on Bleeder Panel inby corner of spad 1830 to inby corner of 1829. The fall was approximately 90ft x 20ft x 8ft. No injuries were reported and no equipment was involved.

Field fire originated on neighbor's property and due to windy conditions, spread onto mine property, burning brush. No injury, or damage to mine equipment. Fire was not extinguished within 30 minutes.

While en-route to rock dust old works foreman identified a roof fall in the roadway at Brk77 1 Brk inby spad 193.Due to the belt line being removed the travel way was re-routed to the old belt entry. The fall was approx. 20'W X 20'L X 4'H the area was supported with 4' fully grouted resin bolts. The fall will not be cleaned up all approaches have been dangered off and timbers set

Accident type, without injuries

High Coal mill inlet temperatures caused and explosion and fire in the coal mill cyclone and dust collector compartments. No personnel were injured and the coal mill' s fire suppression and explosion containment systems functioned properly minimizing property damage.

The 9I Longwall face had a floor bleeder liberating methane in excess of 5%. The operator removed all miners with the exception of those addressing the liberation issue.

Found an unintentional roof fall at Break #61, #6 Heading, parallel with the #6 Beltline. The fall did not affect ventilation or affect the escapeways. The other entries through the area were common to this fall area and were travelable.

Damage to the foot gate controls prevented the mine from raising and lowering personnel with the #3 hoist. The mine still maintained two means of egress with the #1 and #2 hoists.

A SCE pole and transformer were discovered to be the fire, the site ERT and Kern County Fire responded and extinguished to fire, along with SCE. It was believed the fire started from a bird's nest on the pole.

6 CEMEX Engineering project contractors were riding the K3 elevator and it stopped between the second and third floor. Executive Elevator Company was called out to troubleshoot and reset the elevator. The employee exited the elevator at 6:33 PM. There were no injuries as a result of this incident.

An unintentional roof fall occurred on the 1 North 4, Crosscut 52 Entry 5. The roof fall measured approximately 20' in length, 17' in width, and 8' in thickness. A 103 (k) Order (8450328) was issued March 19, 2014, by MSHA Inspector.

The crabapple elevator was not operational for an extended period of time. The door latch motor malfunctioned which faulted the elevator, taking it out of service. No injury as a result.

On March 19, 2014, the power to site failed, tripping out three hoists. Power was restored at 11:51am, and the hoists were reset and functioning by 12:42pm. The power loss occurred during calibration trip units on the main switch gear. It was determined that one of the trip units was incorrectly connected.

A roof fall was discovered on this date. It was located on the 6-North Mains in the #3 entry at crosscut #2 which is in an idle area of the mine. The fall measured 20'X 20' and was 8 feet in thickness. The fall will be cleaned up and resupported. the areas was previously supported with 5' hy-tech bolts and 8' cable bolts. Ventilation nor persons were affected.

Public trash dumpster on mine site. Someone put hot ash in dumpster in a 55 gallon drum. Which cause the trash to ignite. Fire was put out with fire truck and dumpster was flooded with water. Fire was put out at 1300 hours. There were no injuries to any personnel.

Roof Fall A-Mains 1-Blk in Pitt #4 Entry approx. 4'L x 16'W x 4'H

An unintentional roof fall has occurred on the West Submains spad 6838. The fall was approximately 80ft x 18.5ft x 12ft. No injuries were reported and no equipment was involved.

The main conveyance in the Rodeo shaft had malfunctioned, which caused the hoist to stop. There were 12 people on the conveyance when the hoist stopped. There were no injuries in this incident. The problem was identified and fixed a short time later. The 12 employees were brought to surface at approximately 9:35 am that same day.

EE backed loader into a stope & while stepping off noticed flames. He turned off the master switch & activated the fire suppression & evacuated area. The incident cause was an arcing starter cable which melted a hydraulic hose & oil sprayed near the arc causing an ignition. It was determined that it was extinguished immediately & didn't meet immediately reportable criteria.

he 8 haulage elevator, our secondary escapeway, had one of the four auxiliary breaking systems lock into place, causing the elevator to stop movement. The elevator was operational at 5:00pm

Roof Fall in the Third West Submain, in the last open cross-cut in # 8 entry on the #2 unit. The roof fall was about 18 feet wide and about 18 feet long and 7-8 foot high.

On 3/13/2014 a roof fall occurred at 45 break on the 1 Right 1 West in the #3 Entry. This report is being filed due to citation 9112033. The roof fall was called in to the MSHA Hotline on 12/5/2016 to abate the citation. The current operator was not the operator of the mine when the fall occurred. Eastern Associated Coal Company, LLC was the operator at the time of the fall.

The Whittaker elevator stopped with an employee one employee on it. The employee was on the elevator for over 30 minutes. There was no damage to hoisting equipment. There were no compliance or training issues. The elevator was affected by the in coming power supply.

The northeast was engulfed in a winter blizzard. All personnel were leaving the mine early. There was a power outage causing the service hoist to kick out and the high speed breaker on the hoist not to function. It took one and one half hours to get the high speed breaker to function again causing the hoist to be down that long. The service shaft is one of the two escapeways.

A roof fall was found in our P4-2 Panel, x-cut #19, entry #3. The fall occurred in the straights as we were moving the MMU to an intake room set up. The fall was found at 1:55am and phoned into MSHA at 2:08am. The fall did not affect ventilation, impeed passage of miners or cause injury to miners.

A planned roof fall occurred on the active MMU 002 section located at x-cut 31 in the # 2 entry of the S1 M4 headings. Fall measured approximately 20' l x 17' w x 12' h.

The Golden Ridge hoist stopped with 23 employees on it for over 30 minutes. The incident was caused by an outside power issue. The employees were on the hoist for 2.5 hours. It didn't involve any compliance issues. There was no damage to hoisting equipment.

AN UNPLANNED ROOF FALL OCCURRED AT X-CUT #28 IN THE #5 ENTRY ON THE NEW WAY TRACK. THE FALL WAS ABOUT 6 FOOT HIGH, 20 FOOT WIDE, AND 80 FOOT LONG. THE FALL WILL BE CLEANED UP AND REBOLTED BY THE PLAN.

There was no accident injury or illness of persons. A spring cup of one of the brakes mechanism was showing signs of ware and had to be replaced. It took more than 30 min. To trouble shoot and install the hoist was back in production at 2:00pm

Roof fall above anchorage zone B-Mains xc 9 in the #1 entry. No one injured escape way was not affected. Area around the fall was dangered off and breaker rows were set to avoid any additional fall of roof.

At approx. 3:27 PM a roof fall occurred on the 001/002 section. The fall occurred at survey spad 11051 and fell left in the xcut approx. 30' and fell right of the xcut through survey spad 11052 all the way to the wall in the #8 entry.

An unintentional ignition of gas occurred in the #6 entry of the #2 section while the 006-0 MMU was in the process of extracting coal. There were no injuries to miners or damage to equipment during this event.

An unplanned fall of the mine roof at or above the anchorage zone of the installed roof support occurred on the 8 Right longwall set-up face. The fall measured approximately 133' long x 23' wide. The fall was immediately reported to MSHA and WVOMHST.

The Dry Ridge Elevator was not operational for an extended period of time. The Hydraulic unit that holds the rope gripper in the unlocked position was bleeding off. No one was injured as a result of this event

Power Outage in area of mine. Both power feed supplying power to the mine were affected causing both hoist to go down. All miners evacuated via auxiliary power via the man-trip (Koepe) No issues or injuries.

A roof fall was discovered at 11:10 am in the 6th NW Mains (48+00) xc # 66 Entry # 6. The fall was approx 19'length x 19'height x greater than 6'. The outer edge of the fall approx 3' was on a pump# 905 and cable.

Unplanned roof was discovered by the weekly examiner in the 6th NW Submain (46+60) XC # 64 Entry #9. The fall was blocking the examiners travel. Length 30' x Width 19' x greater than 5' height.

The control screen at the top of Dry ridge elevator malfunctioned which caused the brakes to set. There were 13 employees on the elevator at this time. There was no one injured as a result of this incident.

An alleged fire occurred at the dupont raw coal stock pile on the east side. The area was approximately 5 ft by 10 ft. MSHA determined it was a fire and issued a k Order. It didn't involve any compliance issues. No equipment was involved.

Unplanned roof fall occurred on a pump cable, pump # 942 cable. Unknown time or date of fall. Was discovered by weekly examiner. Approx 60' in length x width 18.5' x height greater 6'

The 11 bottom supply man did not close the outer landing gate or securely attach a safety chain during and after unloading a trailer of rock dust from the cage. He lost control of the trailer allowing it to roll back into the shaft falling to the sump below the 9 seam landing. A guide rail was bent causing the elevator to be down until repaired.

There is an old fall about 80' inby this fall. The area outby this old fall was cribbed due to adverse roof. It looks as though this area may fall and the 2 falls will join into 1 fall. Fall dimensions are 80'L X 18'W X 6'H. Fall area SW Mains #3 #10 entry spad #6863

The Crabapple elevator was in-operational for a period of time due to a broken door switch on the elevator door.

Blown fuse caused by east compressor. The mine was evacuated at 5:17 pm and completed at 5:46 pm. The hoist was repaired and tested. Miners returned underground at 9:50 pm.

DURING A WEEKLY EXAM THE FIRE BOSS DISCOVERED A ROOF FALL IN THE 1 RIGHT OFF OF 2 EAST PANEL. THE FALL IS LOCATED IN # 4 ENTRY THREE BRKS. INBY SPAD # 2543.

At approximately 6:50am the Schindler elevator became inoperable. Dirt was found on the door re-opener photo eyes therefore the automatic doors would not shut. The safe edge was cleaned and the elevator was back in operation.

Roof fall out-by, neutral entry , aprox 32 breaks out-by face area. Ventilation was not disrupted. Fall occurred during idle weekend.

A gate on the North Man/Material hoist was not completely shut and came open during operation shutting the hoist down in the shaft. There was not any miners on the hoist during this failure. All affected miners were withdrawn out of the mine via our south slope.

The fan was rubbing the guard on the hoist motor. They took the guard off and put a set screw on the fan which twisted off. Hoist was operational at 3:10 pm.

At 6:42 the yard man noticed some liners on the yard. He reported to the hoist man witch immediately shut the hoist down. Inspected the hoist and found the liners had came out of the lower sheave wheel. We elected to keep the hoist down. Mechanical it could run and safely. We did not want to take a chance on damaging our ropes or slinging any other liners out.

We had a tailgate blockage at the tail of the longwall section between x-cut #89 and #90 in number 3 entry of the Tailgate.

Struck by falling object

Excavator was lifting plastic pipe when it fell and hit the victim.

Caught in, under or between a moving and a stationary object

The victim was in the process of cutting through the inner left-side side plate. When the cut was completed, the cat frame pivoted upward, pinning the victim between the cat track and frame of the feeder. The side plates connect the hopper jack assembles to the cat frame. The victim was working on n Auxier Welding Inc belt feeder.

Caught in, under or between a moving and a stationary object

Employee was changing jaw dies & had his hand between a crescent wrench & a piece of angle iron. Employee asked another employee to kick the end of the wrench to break the nut free. When the employee kicked it the nut spun quick & the employee's left ring finger was smashed between the angle iron & the crescent wrench. The tip of the employee's left ring finger was amputated.

While attempting to free stuck trough roller employee caught finger between roller and belt.

Over-exertion in lifting objects

Employee was repositioning (on a pallet) a railroad car brake beam that had been previously loaded with chain onto the pallet. Employee felt an abdominal strain the next day.

Caught in, under or between a moving and a stationary object

The Compliance Staffing Agency contractor was moving a dolly at the longwall storage unit. He had his hand on the drop off ear. When the dolly ear came in contact with the storage unit drop off arm, his hand got pinched between them. The tip of his finger was removed.

Caught in, under or between a moving and a stationary object

Two employees were removing shafts on a feeder pan housing at the primary. While pulling out one of the shafts, the injured employee put his hand between the chain of a come-a-long and the feeder frame, smashing his 5th right finger. He sought medical attention, resulting in about 1/4 inch of bone amputation of his 5th right finger.

Broke a leg cut off the other

Over-exertion in lifting objects

Employee called in from home and stated that this condition had been bothering him for years. Employee thought he had a pulled a groin. Sent him from home to doctor. He chose to go to the emergency room. Attending Physician examined and diagnosed a hernia. Physician recommended that he see his personal doctor.

Employee was backing carrier into #3 entry at tailpiece. As he was backing around tailpiece a piece of rock busted from the bottom striking him on his left foot. Injury ultimately required partial amputation of the injured foot.

Over-exertion (Not Elsewhere Classified)

Employee was climbing the ladder to enter the CAT 777 haul truck when his foot slipped off the step which left him hanging from the grab rail with his right arm. He continued to work until 04/09/2014. At this time he went to see a physician and is now off work for at least 2 weeks. He will be receiving occupational therapy for a right shoulder strain.

Miner was removing a sensor above his head when he felt a pain in his neck

Struck by falling object

The employee was rockdusting an entry with a power duster. He rockdusted the roof and a piece of stone fell out of the roof striking his hand.

Fall from machine

Climbing out of the Mechanics Service Truck. Stepped on Ladder, ladder feel to one side, causing employee to fall. Landed between rungs of ladder; then folded up on employees left leg causing 1 bone to break and another bone to fracture.

Employee was standing on top of supply car and when the employee stepped off of the car he slipped twisting ankle causing sprain to ankle.

Struck by falling object

EMPLOYEE WAS REMOVING THE CARRIER BEARING/PARKING BRAKE ASSEMBLY FROM UNIT 1580 WHEN IT FELL ON HIS RIGHT ANKLE.

Over-exertion in lifting objects

Employee was moving continuous miner cable out of the entry in preparation for mining. When he lifted the cable to his waist he felt a sharp pain between his shoulder blades and shot down his back. He went down on the ground and had lost the feeling from his shoulders down. No physical damage has been identified by the medical testing.

Struck against a moving object

While driving a 789 haul truck the seat belt contacted the seat recline control causing the seat to unexpectedly recline.

Caught in, under or between a moving and a stationary object

7 employees were setting-up a spill board on the belt tail when the spill board suddenly dropped and caught the employee's fingers.

Over-exertion in lifting objects

While lifting sand jacks and pushing against top to let sand drain down, Employee felt a sharp pain in his lower back. Employee was treated at Raleigh General Hospital.

Employee after taken to the ER the employee has a fractured bone in the hip.

Preparing barge for loading, movement of barge caused EE to lose balance, falling approx. 12' into empty barge. Ladder was provided & EE was to climb out of barge. EE sustained fractures to L wrist, R hip & lacerations to forehead. 7000-1 report is being filed as an excise of caution and does not constitute an admission that MSHA has jurisdiction over this injury.

The Employee was operating an off standard center drive shuttle car, after being loaded by the continuous miner, the employee swung around to the inby seat with out setting his parking brake. When the Employee was swinging around to the inby seat, the shuttle car started rolling and caught his right leg against the rib. The result was a fractured tibia.

He injured his left knee on the metal track while falling off the back of excavator around 6.5 feet below. went to the hospital where they said it was a sprained knee. Did not return to work on 3/24/2014 making it reportable.

Injured employee stated he struck his head on the mine roof while walking to a portal bus at the start of the shift resulting in a strained neck. Later in the shift, the employee again struck his head, on a continuous miner cable on the section.

EE was loading 50lb. prell bags from a tractor trailer onto the powder truck in preparation for loading headings. When he lifted one of the bags he stated that he felt pain in his lower back. EE was diagnosed with a strained back and did not work March 25, 26, & 27.

Employee and 2 other employee's were moving a water pump from a muddy area that had already been pumped down to another location. The pump weighed approximately 75 pounds but muddy conditions made moving the pump more difficult. Later that day employee began experiencing pain in his lower back and down his right leg. EE complained of a sore left knee with swelling; stated he wasn't sure what from but that he was kneeling on steel balls while lining a primary mill. EE was evaluated by a Physician and underwent surgery on 7-24-14 resulting in LT.

Employee alleges that he lifted a pallet and he felt his left shoulder pop. This employee has had surgery on both shoulders for off job injuries. Management is waiting for conclusive medical evidence from MRI to substantiate whether or not a new injury has occurred.

The mantrip operator intentionally drove into a track switch to enter the switch, the mantrip climbed the rail and went off track causing the injured to hit his hardhat on the roof causing pain in his neck and back. Employee was helping a mechanic tighten a loose hose connection in the back of a lube truck when the mechanic bumped the truck rear door lever causing the door to come down and strike the employee across the back area resulting in two (2) broken ribs.

The event became reportable when the ee began missing work on 3-17,2014. The injury occurred because the ee was not paying attention to his surroundings. During battery change-out the lead was pulled out of the plug on the Scoop. Following repairs of the cable and plug the plug was connected to the jumper lead to the fresh battery. Operations was in the process of relocating a de-energized power cable. Employees were positioned as spotters as a backhoe was used to pull the cable. One employee was on the opposed side of the cable from the others. As cable was pulled it became hung up on a berm. As employees signaled for the backhoe to stop the cable broke through the berm and struck employee at the knee.

Employee was relieving the pressure on the lime slurry injection system the discharge was plugged of the relief valve. Employee thought the pressure was gone and opened the line when it hot lime water shot out and went up his shirt sleeve burning his left arm.

The employee was coming off a dozer and twisted his ankle.

Fall down stairs

03/17/14 Miner reported falling while descending screen house stairs, pain to right knee. 04/03/14 Miner requested to see doctor, released full duty. 05/02/14 Miner had follow up doctor apt. 06/12/14 Miner saw orthopedic doctor. Company was not notified of specialist 06/12/13 until 08/04/14. Injury meets reportable status on 06/12/14.

Caught in, under or between a moving and a stationary object

Employee Shifting Pan Feeder tracks and one slipped which smashed his hand causing fractured finger tips on ring and middle fingers of left hand.

Struck against stationary object

Employee was moving a roof bolt strap, while pulling one strap his wrist hit another strap cutting it. Injury required five stitches.
